The coronal unicondylar fracture of the distal femur (AO 33-B3) is a rare intraarticular injury within the weight bearing area of the knee, initially described by Albert Hoffa in 1904. We report an unusual combination of a Hoffa fracture with lateral patellar dislocation in a young adult. Our patient sustained the injury by a sudden twist of his leg during sports. He presented clinically with knee swelling, dislocation of the patella, and localized tenderness; unable to bare weight. After plane radiograph confirmed the injury, manual reduction of the patella was done by hyperextension of the knee and medialward pressure. Afterwards, a CT scan and MRI were conducted. The injury was surgically treated with lag-screws, locking-plate and MPFL-reconstruction.
[Purpose] A wide variety of accelerometer tools are used to estimate human movement, but there are no adequate data relating to gait symmetry parameters in the context of knee osteoarthritis. This study's purpose was to evaluate a 3D-kinematic system using body-mounted sensors (gyroscopes and accelerometers) on the trunk and limbs. This is the first study to use spectral analysis for data post processing. [Subjects] Twelve patients with unilateral knee osteoarthritis (OA) (10 male) and seven age-matched controls (6 male) were studied. [Methods] Measurements with 3-D accelerometers and gyroscopes were compared to video analysis with marker positions tracked by a six-camera optoelectronic system (VICON 460, Oxford Metrics). Data were recorded using the 3D-kinematic system. [Results] The results of both gait analysis systems were significantly correlated. Five parameters were significantly different between the knee OA and control groups. To overcome time spent in expensive post-processing routines, spectral analysis was performed for fast differentiation between normal gait and pathological gait signals using the 3D-kinematic system. [Conclusions] The 3D-kinematic system is objective, inexpensive, accurate and portable, and allows long-term recordings in clinical, sport as well as ergonomic or functional capacity evaluation (FCE) settings. For fast post-processing, spectral analysis of the recorded data is recommended.
Background
For improved outcomes in total knee arthroplasty (TKA) correct implant fitting and positioning are crucial. In order to facilitate a best possible implant fitting and positioning patient-specific systems have been developed. However, whether or not these systems allow for better implant fitting and positioning has yet to be elucidated. For this reason, the aim was to analyse the novel patient-specific cruciate retaining knee replacement system iTotal (TM) CR G2 that utilizes custom-made implants and instruments for its ability to facilitate accurate implant fitting and positioning including correction of the hip-knee-ankle angle (HKA).
Methods
We assessed radiographic results of 106 patients who were treated with the second generation of a patient-specific cruciate retaining knee arthroplasty using iTotal\(^{TM}\) CR G2 (ConforMIS Inc.) for tricompartmental knee osteoarthritis (OA) using custom-made implants and instruments. The implant fit and positioning as well as the correction of the mechanical axis (hip-knee-ankle angle, HKA) and restoration of the joint line were determined using pre- and postoperative radiographic analyses.
Results
On average, HKA was corrected from 174.4 degrees +/- 4.6 degrees preoperatively to 178.8 degrees +/- 2.2 degrees postoperatively and the coronal femoro-tibial angle was adjusted on average 4.4 degrees. The measured preoperative tibial slope was 5.3 degrees +/- 2.2 degrees (mean +/- SD) and the average postoperative tibial slope was 4.7 degrees +/- 1.1 degrees on lateral views. The joint line was well preserved with an average modified Insall-Salvati index of 1.66 +/- 0.16 pre- and 1.67 +/- 0.16 postoperatively. The overall accuracy of fit of implant components was decent with a measured medial overhang of more than 1 mm (1.33 mm +/- 0.32 mm) in 4 cases only. Further, a lateral overhang of more than 1 mm (1.8 mm +/- 0.63) (measured in the anterior-posterior radiographs) was observed in 11 cases, with none of the 106 patients showing femoral notching.
Conclusion
The patient-specific iTotal\(^{TM}\) CR G2 total knee replacement system facilitated a proper fitting and positioning of the implant components. Moreover, a good restoration of the leg axis towards neutral alignment was achieved as planned. Nonetheless, further clinical follow-up studies are necessary to validate our findings and to determine the long-term impact of using this patient- specific system.
Neurotransmitter release is stabilized by homeostatic plasticity. Presynaptic homeostatic potentiation (PHP) operates on timescales ranging from minute- to life-long adaptations and likely involves reorganization of presynaptic active zones (AZs). At Drosophila melanogaster neuromuscular junctions, earlier work ascribed AZ enlargement by incorporating more Bruchpilot (Brp) scaffold protein a role in PHP. We use localization microscopy (direct stochastic optical reconstruction microscopy [dSTORM]) and hierarchical density-based spatial clustering of applications with noise (HDBSCAN) to study AZ plasticity during PHP at the synaptic mesoscale. We find compaction of individual AZs in acute philanthotoxin-induced and chronic genetically induced PHP but unchanged copy numbers of AZ proteins. Compaction even occurs at the level of Brp subclusters, which move toward AZ centers, and in Rab3 interacting molecule (RIM)-binding protein (RBP) subclusters. Furthermore, correlative confocal and dSTORM imaging reveals how AZ compaction in PHP translates into apparent increases in AZ area and Brp protein content, as implied earlier.
Background:
The integrity of the flexor tendon pulley apparatus is crucial for unimpaired function of the digits. Although secondary reconstruction is an established procedure in multi-pulley injuries, acute reconstruction of isolated, closed pulley ruptures is a rare occurrence. There are 3 factors influencing the functional outcome of a reconstruction: gapping distance between tendon and bone (E-space), bulkiness of the reconstruction, and stability. As direct repair is rarely done, grafts are used to reinforce the pulley. An advantage of the first extensor retinaculum graft is the synovial coating providing the possibility to be used both as a direct graft with synovial coating or as an onlay graft after removal of the synovia when the native synovial layer is present.
Methods:
A graft from the first dorsal extensor compartment is used as an onlay graft to reinforce the sutured A4 pulley. This technique allows reconstruction of the original dimensions of the pulley system while stability is ensured by anchoring the onlay graft to the bony insertions of the pulley.
Results:
Anatomical reconstruction can be achieved with this method. The measured E-space remained 0 mm throughout the recovery, while the graft incorporated as a slim reinforcement of the pulley, displaying no bulkiness.
Conclusions:
The ideal reconstruction should provide synovial coating and sufficient strength with minimal bulk. Early reconstruction using an onlay graft offers these options. The native synovial lining is preserved and the graft is used to reinforce the pulley.

Hintergrund
Die Versorgung von Patellafrakturen ist technisch anspruchsvoll. Auch wenn die radiologischen Ergebnisse zumeist zufriedenstellend sind, deckt sich dies häufig nicht mit der subjektiven Einschätzung der Patienten. Die klassische Versorgung mittels Drahtzuggurtung weist einige Komplikationen auf. Die winkelstabile Plattenosteosynthese hat sich in den letzten Jahren biomechanisch als vorteilhaft erwiesen.
Fragestellung
Von wem werden Patellafrakturen in Deutschland versorgt? Wie sieht der aktuelle Versorgungsstandard aus? Haben sich „moderne“ Osteosyntheseformen durchgesetzt? Was sind die häufigsten Komplikationen?
Material und Methoden
Die Mitglieder der Deutschen Gesellschaft für Orthopädie und Unfallchirurgie sowie der Deutschen Kniegesellschaft wurden aufgefordert, an einer Onlinebefragung teilzunehmen.
Ergebnisse
Insgesamt wurden 511 komplett ausgefüllte Fragebogen ausgewertet. Die Befragten sind zum größten Teil auf Unfallchirurgie spezialisiert (51,5 %) und verfügen über langjährige Berufserfahrung in Traumazentren. Die Hälfte der Operateure versorgt ≤5 Patellafrakturen jährlich. In knapp 40 % der Fälle wird die präoperative Bildgebung um eine Computertomographie ergänzt. Die klassische Zuggurtung ist noch die bevorzugte Osteosyntheseform bei allen Frakturtypen (Querfraktur 52 %, Mehrfragmentfrakturen 40 %). Bei Mehrfragmentfrakturen entscheiden sich 30 % der Operateure für eine winkelstabile Plattenosteosynthese. Bei Beteiligung des kaudalen Pols dient als zusätzliche Sicherung die McLaughlin-Schlinge (60 %).
Diskussion
Der Versorgungsstandard von Patellafrakturen in Deutschland entspricht weitgehend der aktualisierten S2e-Leitlinie. Nach wie vor wird die klassische Zuggurtungsosteosynthese als Verfahren der Wahl genutzt. Weitere klinische (Langzeit‑)Studien werden benötigt, um die Vorteile der winkelstabilen Plattenosteosynthese zu verifizieren.
Die Anzahl an Fahrradfahrern steigt in allen Altersgruppen. Mit zunehmender Popularität erhöht sich die Anzahl an Unfällen mit zum Teil schweren Verletzungen. Im Zuge dessen stellt sich die Frage, welchen Einfluss das Alter auf die Art und Schwere der Verletzungen, die Überlebenswahrscheinlichkeit und die Krankenhausverweildauer bei schwerverletzten Fahrradfahrern hat.
Methoden: Es wurde eine retrospektive Auswertung der Daten des TraumaRegisters DGU® der Jahre 2010-2019 durchgeführt. Alle schwerverletzten Fahrradfahrer mit einem MAIS 3+ (N=14.651) im TR-DGU wurden in diese Studie eingeschlossen und die vorliegenden Parameter ausgewertet. Es erfolgte eine Unterteilung in vier Altersgruppen (20 - 59, 60 - 69, 70 - 79 und ≥ 80 Jahre).
Ergebnisse: Verletzungen des Schädels traten mit 64,2% mit Abstand am häufigsten auf. Es zeigte sich eine deutliche Zunahme der schweren Kopfverletzungen in der Gruppe der über 60-Jährigen. Mit steigendem Alter nahm des Weiteren die Wahrscheinlichkeit einer präklinischen Intubation, die Katecholaminpflichtigkeit, die Intensiv- und Krankenhausverweildauer sowie die Sterblichkeit zu.
Schlussfolgerung: Kopfverletzungen stellen die häufigste schwere Verletzung bei Fahrradfahrern dar. Da das Helmtragen nicht erfasst wird kann auf dessen Effektivität kein Rückschluss gezogen werden. Ein höheres Alter korreliert des Weiteren mit einer höheren Sterblichkeit, stellt jedoch keinen unabhängigen Risikofaktor zum Versterben bei einem schwerverletzten Patienten dar.
Das Motorradfahren erfreut sich in der heutigen Gesellschaft nach wie vor einer großen Beliebtheit. Es ist jedoch unbestritten, dass es sich um ein sehr gefährliches Fortbewegungsmittel handelt. Motorradfahrer haben ein deutlich größeres Risiko von schweren Verletzungen betroffen zu sein oder tödlich zu verunglücken als andere Benutzer von Kraftfahrzeugen. Von insgesamt rund 400.000 Verletzten bei Straßenverkehrsunfällen im Jahr 2016 verunglückten ca. 11% (rund 44.000 Verletzte) auf einem Motorrad. Von insgesamt 3.206 Todesopfern waren 604 Motorradfahrer (ca. 19%). Insbesondere die Analyse des bestandsbezogenen Risikos zeigt, was allgemein vermutet wird: Das Kraftradfahren birgt größere Risiken als das Autofahren.
Je 100.000 Fahrzeuge wurden 2016 bei Straßenverkehrsunfällen 16 Benutzer von Krafträdern mit amtlichen Kennzeichen und Krafträdern mit Versicherungskennzeichen tödlich verletzt, während je 100.000 zugelassener Fahrzeuge nur 3 PKW-Insassen tödlich verunglückten.
Das bestandsbezogene Risiko auf einem Kraftrad tödlich zu verunglücken war im Jahr 2016 also mehr als fünf Mal höher als in einem PKW.
Unfälle mit einem motorisierten Zweirad sind häufig assoziiert mit Verletzungen multipler anatomischer Regionen und machen das Opfer eines solchen Unfalls zu einem komplexen Traumapatienten, der einer adäquaten Einschätzung und eines
entsprechenden Managements bedarf. Im klinischen Alltag fällt auf, dass vermehrt „ältere“ Patienten mit dem Motorrad verunfallen und das behandelnde medizinische Personal vor Schwierigkeiten stellen, da aufgrund einer größeren Anzahl an Komorbiditäten der Gesundheitszustand nicht dem jüngerer Menschen entspricht. Analog hierzu zeigten sich bei der Auswertung der verunglückten Motorradfahrer des TraumaRegister DGU® zwei Altersgipfel.
Die vorliegende altersbezogene Analyse von polytraumatisierten Motorradfahrern anhand eines deutschen, überregionalen Traumazentrums und des TraumaRegister DGU® wurde durchgeführt, um die besonderen Herausforderungen in der Versorgung jüngerer und älterer schwerverletzter Motorradfahrer herauszuarbeiten und ihr Verletzungsmuster zu analysieren.
Im Rahmen der Arbeit konnte gezeigt werden, dass die Versorgung älterer schwerverletzter Motorradunfallopfer eine besondere Herausforderung darstellt. Trotz nahezu gleicher anatomischer Verletzungsschwere nach ISS konnten wir belegen, dass höheres Alter im Rahmen eines Motorradunfalls, aufgrund eines ungünstigeren biologischen Risikoprofils durch prätraumatisch mehr chronische Begleiterkrankungen, mit einer erhöhten Morbidität, Mortalität und einer deutlich aufwendigeren medizinischen Versorgung einhergeht. Im Patientengut des TR-DGU fiel vor allem auf, dass die Kohorte der > 65-jährigen Opfer eines Motorradunfalls ein im Vergleich zur Allgemeinbevölkerung erhöhtes Sterblichkeitsrisiko hat. Des Weiteren konnte herausgearbeitet werden, dass ältere Unfallopfer erst im Verlauf des Krankenhausaufenthalts tödliche Komplikationen zu entwickeln scheinen, während jüngere Patienten den Komplikationen des direkten Unfalltraumas früher erliegen.
Es zeigte sich, dass im Rahmen eines Motorradunfalls mit einem breit gefächerten Verletzungsmuster zu rechnen ist. So wurden bei den 70 eingeschlossenen Unfallopfern insgesamt 309 Verletzungen diagnostiziert, was einem Durschnitt von 4,4 Verletzungen pro verunglücktem Motorradfahrer entspricht. Als besonders anfällig für Verletzungen erwiesen sich die Extremitäten und der Thorax. Insbesondere die oftmals lebensbedrohlichen schweren Thoraxverletzungen betrafen in beiden Patientenkollektiven signifikant häufiger ältere Opfer von Motorradunfällen. Kopfverletzungen hingegen, am häufigsten für den tödlichen Ausgang eines Motorradunfalls verantwortlich, betrafen in beiden Kollektiven tendenziell häufiger jüngere Patienten.
Es sollten in Zukunft weiterhin präventive Maßnahmen erforscht werden, um dem nachweislich erhöhten Risiko eines Motorradunfalls adäquat begegnen zu können. Gegenstand zukünftiger Forschung kann es außerdem sein, das Thoraxtrauma und sekundär eintretende Komplikationen im Rahmen eines Motorradunfalls zu verringern. Dies ist insbesondere für ältere Motorradfahrer über 65 Jahre von grundlegender Bedeutung.
Die fettige Degeneration (FD) der Rotatorenmanschettenmuskulatur beeinflusst das funktionelle und anatomische Ergebnis nach der Rotatorenmanschettenrekonstruktion. Die MRT-basierende Abschätzung der fettigen Degeneration ist der aktuelle Goldstandard im klinischen Alltag. Es gibt Hinweise darauf, dass die Ultraschallelastographie (EUS) lokale Unterschiede der Gewebesteifigkeit in Muskeln und Sehnen feststellen kann. Mit der Scherwellenelastographie (SWE) wurde versucht zu bestimmen, in welchem Ausmaß die Scherwellengeschwindigkeit mit den Messungen der Fettentartung verbunden war. Die MRT-spektroskopische Fettmessung wurde als Referenz verwendet, um die Fettmenge im Muskelbauch zu quantifizieren.
METHODEN:
Bei 42 Patienten wurde die SWE am dicksten Durchmesser des Supraspinatusmuskels angewendet. Anschließend wurde eine MRT-spektroskopische Fettmessung des Supraspinatusmuskels mit der SPLASH-Technik durchgeführt. Eine gelgefüllte Kapsel wurde verwendet, um die gemessene Fläche im MRT zu lokalisieren. Die mit der SWE und der spektroskopischen Fettmessung gemessenen Werte der Scherwellengeschwindigkeit wurden unter Verwendung des Pearson-Korrelationstests statistisch korreliert.
ERGEBNISSE:
Die Korrelation der mit der MRT-Spektroskopie gemessenen Fettmenge und der mit der SWE gemessenen Scherwellengeschwindigkeit betrug p = 0,82. Das spektroskopisch gemessene Fettverhältnis des Supraspinatusmuskels lag zwischen 0% und 77,41% und der Scherwellengeschwindigkeit zwischen 1,59 m / s und 5,32 m / s. Bei 4 Patienten konnte keine ausreichende Scherwellenelastographie durchgeführt werden. Diese Personen wiesen einen größeren Durchmesser des darüber liegenden Weichgewebes auf. Die mit SWE gemessene Scherwellengeschwindigkeit zeigte eine gute Korrelation mit der MRT-spektroskopischen Fettmenge des Supraspinatusmuskels.
FAZIT:
Diese vorläufigen Daten deuten darauf hin, dass SWE eine gute Methode zum Erkennen und Abschätzen der fettigen Degeneration im Supraspinatusmuskel in Echtzeit sein kann.
