Background
For improved outcomes in total knee arthroplasty (TKA) correct implant fitting and positioning are crucial. In order to facilitate a best possible implant fitting and positioning patient-specific systems have been developed. However, whether or not these systems allow for better implant fitting and positioning has yet to be elucidated. For this reason, the aim was to analyse the novel patient-specific cruciate retaining knee replacement system iTotal (TM) CR G2 that utilizes custom-made implants and instruments for its ability to facilitate accurate implant fitting and positioning including correction of the hip-knee-ankle angle (HKA).
Methods
We assessed radiographic results of 106 patients who were treated with the second generation of a patient-specific cruciate retaining knee arthroplasty using iTotal\(^{TM}\) CR G2 (ConforMIS Inc.) for tricompartmental knee osteoarthritis (OA) using custom-made implants and instruments. The implant fit and positioning as well as the correction of the mechanical axis (hip-knee-ankle angle, HKA) and restoration of the joint line were determined using pre- and postoperative radiographic analyses.
Results
On average, HKA was corrected from 174.4 degrees +/- 4.6 degrees preoperatively to 178.8 degrees +/- 2.2 degrees postoperatively and the coronal femoro-tibial angle was adjusted on average 4.4 degrees. The measured preoperative tibial slope was 5.3 degrees +/- 2.2 degrees (mean +/- SD) and the average postoperative tibial slope was 4.7 degrees +/- 1.1 degrees on lateral views. The joint line was well preserved with an average modified Insall-Salvati index of 1.66 +/- 0.16 pre- and 1.67 +/- 0.16 postoperatively. The overall accuracy of fit of implant components was decent with a measured medial overhang of more than 1 mm (1.33 mm +/- 0.32 mm) in 4 cases only. Further, a lateral overhang of more than 1 mm (1.8 mm +/- 0.63) (measured in the anterior-posterior radiographs) was observed in 11 cases, with none of the 106 patients showing femoral notching.
Conclusion
The patient-specific iTotal\(^{TM}\) CR G2 total knee replacement system facilitated a proper fitting and positioning of the implant components. Moreover, a good restoration of the leg axis towards neutral alignment was achieved as planned. Nonetheless, further clinical follow-up studies are necessary to validate our findings and to determine the long-term impact of using this patient- specific system.
Ischemia-like stress conditions stimulate trophic activities of adipose-derived stromal/stem cells

Adipose-derived stromal/stem cells (ASCs) have been shown to exert regenerative functions, which are mainly attributed to the secretion of trophic factors. Upon transplantation, ASCs are facing an ischemic environment characterized by oxygen and nutrient deprivation. However, current knowledge on the secretion capacity of ASCs under such conditions is limited. Thus, the present study focused on the secretory function of ASCs under glucose and oxygen deprivation as major components of ischemia. After exposure to glucose/oxygen deprivation, ASCs maintained distinct viability, but the metabolic activity was greatly reduced by glucose limitation. ASCs were able to secrete a broad panel of factors under glucose/oxygen deprivation as revealed by a cytokine antibody array. Quantification of selected factors by ELISA demonstrated that glucose deprivation in combination with hypoxia led to markedly higher secretion levels of the angiogenic and anti-apoptotic factors IL-6, VEGF, and stanniocalcin-1 as compared to the hypoxic condition alone. A conditioned medium of glucose/oxygen-deprived ASCs promoted the viability and tube formation of endothelial cells, and the proliferation and migration of fibroblasts. These findings indicate that ASCs are stimulated by ischemia-like stress conditions to secrete trophic factors and would be able to exert their beneficial function in an ischemic environment.
In der Plastischen Chirurgie erfordert die Rekonstruktion von ästhetisch anspruchsvollen Bereichen in vielen Fällen die Wiederherstellung von subkutanem Fettgewebe. Neben chirurgischen Rekonstruktionen könnte das Tissue Engineering von Fettgewebe einen wertvollen Beitrag leisten. Jedoch bringt es vielschichtige Herausforderungen mit sich und ist zum aktuellen Zeitpunkt nur limitiert möglich. Ein Ansatz ist die Schaffung einer Trägermatrix zur Besiedelung und Differenzierung von Stammzellen. Auf dieser Basis sollten in der vorliegenden Arbeit zwei Teilbereiche untersucht werden. In dem ersten Teilbereich erfolgten Untersuchungen verschiedener Gewinnungsmethoden von ASCs aus dem subkutanen Fettgewebe bezogen auf ihr Effizienz. Die untersuchten Liposuktionstechniken zeigten eine deutlich höhere Effizienz gegenüber der mechanischen Gewinnungsmethode bezogen auf die gewonnene Zellzahl. In den Viabilitätsuntersuchungen zeigte sich eine ähnliche Tendenz. ASCs aller drei Gewinnungsmethoden proliferierten durchaus gleich gut, jedoch zeigten die histologischen und quantitativen Adipogeneseuntersuchungen tendenziell mehr Lipidbildung bei den Liposuktionstechniken.
Das übergeordnete Ziel des zweiten Abschnittes dieser Arbeit war es eine Trägermatrix auf Hyaluronsäure-Basis mit dem vielseitig modifizierbarem Crosslinker Polyglycidol zu untersuchen, sie mit mesenchymalen Stammzellen aus dem Fettgewebe zu besiedeln und diese adipogen zu differenzieren. Des Weiteren erfolgten erste Versuche die Hydrogele mit funktionellen Gruppen zu modifizieren um eine Verbesserung der Adhäsion der Zellen im Hydrogel zu erreichen. Die unmodifizierten Hydrogele waren zu jeder Zeit stabil in ihrer Form und zeigten nach Besiedelung mit ASCs eine gleichmäßige Verteilung der Zellen im Gel. Auch ließ sich die Adipogenese histologisch visualisieren und biochemisch bestätigen. Die inkorporierten Peptide brachten eine peptidabhängige und konzentrationsabhängige Veränderung der Zellverteilung im Hydrogel. Eine Steigerung der Funktionalität der Zellen bezogen auf das Überleben und die Adipogenese konnte in diesen ersten Versuchen noch nicht gezeigt werden.
Generell zeigt sich eine Eignung der hyaluronsäurebasierten mit Polyglycidol-verlinkten Hydrogele für das Tissue Engineering von Fettgewebe. Weitere Untersuchungen bezüglich der Modifikation der Hydrogele mit adhäsiven und adipogenen funktionellen Gruppen bietet sich daher an und könnte ein fettgewebsähnliches Umgebungsmilieu hervorbringen.
In dieser Studie wird die biomechanische Stabilität des anterioren subkutanen Fixateur Interne (USS II Polyaxiales- bzw. Click’X System) zur Stabilisierung der vorderen Beckenringverletzung mit der des supraacetabulären Fixateur Externe verglichen.
An 36 Beckenmodellen aus Polyurethanschaum wurden instabile Beckenringverletzungen generiert. Diese wurden zunächst posterior mit je zwei sakroiliakalen Schrauben und anterior jeweils mit einem der drei genannten Fixateure versorgt und dann im Einbeinstand für je 1000 Zyklen mit 300, 400 und 500 N be- und entlastet.
Statistisch zeigt sich zwischen den drei Gruppen kein signifikanter Unterschied für die Stabilität.
Mit geringeren Komplikationsraten und vergleichbarer Stabilität bietet der anteriore subkutane Fixateur Interne klinisch sowie biomechanisch eine Alternative zum Fixateur Externe.
Zudem besteht die Forderung nach einem für die Beckenringfixation konzipierten subkutanen Fixations-System, das unerwünschte Bewegungsfreiheiten des Titanbügels verhindert und damit Risiken für die Patienten minimiert.
Nicotinamide N-methyltransferase (NNMT) is a new regulator of energy homeostasis. Its expression is increased in models of obesity and diabetes. An enhanced NNMT level is also caused by an adipose tissue-specific knockout of glucose transporter type 4 (GLUT4) in mice, whereas the overexpression of this glucose transporter reduced the NNMT expression. Furthermore, the knockdown of the enzyme prevents mice from diet-induced obesity (DIO) and the recently developed small molecule inhibitors for NNMT reverses the DIO. These previous findings demonstrated the exclusive role of NNMT in adipose tissue and further make it to a promising target in obesity treatment. However, the regulation mechanism of this methyltransferase is not yet clarified.
The first part of the thesis focus on the investigation whether pro-inflammatory signals are responsible for the enhanced NNMT expression in obese adipose tissue because a hallmark of this tissue is a low-level chronic inflammation. Indeed, the NNMT mRNA in our study was elevated in obese patients compared with the control group, whereas the GLUT4 mRNA expression does not differ between lean and obese humans. To analyze whether pro inflammatory signals, like interleukin (IL 6) and tumor necrosis factor α (TNF-α), regulate NNMT expression 3T3-L1 adipocytes were treated with these cytokines. However, IL 6, TNF α, and leptin, which is an alternative activator of the JAK/STAT pathway, did not affect the NNMT protein or mRNA level in differentiated 3T3-L1 adipocytes. The mRNA and protein levels were measured by quantitative polymerase chain reaction (qPCR) and western blotting.
In the second part of this study, 3T3-L1 adipocytes were cultivated with varying glucose concentrations to show whether NNMT expression depends on glucose availability. Further studies with activators and inhibitors of AMP-activated protein kinase (AMPK) and mechanistic target of rapamycin (mTOR) signaling pathways were used to elucidate the regulation mechanism of the enzyme.
The glucose deprivation of differentiated 3T3-L1 adipocytes led to a 2-fold increase in NNMT expression. This effect was confirmed by the inhibition of the glucose transports with phloretin as well as the inhibition of glycolysis with 2-deoxyglucose (2-DG). AMPK serves as an intracellular energy sensor and the pharmacological activation of it enhanced the NNMT expression. This increase was also caused by the inhibition of mTOR. Conversely, the activation of mTOR using MHY1485 prevented the effect of glucose deprivation on NNMT. Furthermore, the NNMT up-regulation was also blocked by the different autophagy inhibitors.
Taken together, NNMT plays a critical role in autophagy in adipocytes, because an inhibition of this process prevented the augmented NNMT expression during glucose starvation. Moreover, the effect on NNMT protein and mRNA level depends on AMPK and mTOR. However, pro-inflammatory signals did not affect the expression. Further in vivo studies have to clarify whether AMPK activation and mTOR inhibition as well as autophagy are responsible for the increased NNMT levels in obese adipose tissue. In future this methyltransferase emerges as an awesome therapeutic target for obesity.
In einem Zeitraum von 8 Jahren (01.01.2008 – 31.12.2015) wurden 51 Patienten am Universitätsklinikum Würzburg mit einer dorsalen Plattenosteosynthese bei distaler Radiusfraktur versorgt. Von diesen 51 Patienten konnten durchschnittlich 5 ± 2 Jahre postoperativ ( 1,4 – 9,2 Jahre) 90 % (46 Patienten) der Patienten nachuntersucht werden. Es wurden nur 6 % aller Patienten die eine Plattenosteosynthesen bei distaler Radiusfraktur innerhalb von 8 Jahren (01.01.2008 – 31.12.2015) am Universitätsklinikum Würzburg erhalten haben von dorsal versorgt. 94% der Plattenosteosynthesen bei distaler Radiusfraktur erfolgten von palmar. Zu Diesem Patientenkollektiv wurde ebenfalls eine klinische Studie mit den gleichen Auswertungsmethoden, Fragebögen und Untersuchungsgeräten wie in unserer Studie durchgeführt. Die niedrige Fallzahl in diesem Zeitraum ist auf die seltene Indikationsstellung zur dorsalen Plattenosteosynthese zurückzuführen. Die Indikation wurde am Universitätsklinikum Würzburg fast ausschließlich computertomographisch präoperativ festgelegt und lag vor bei einer zentralen Gelenksimpression, dorsaler Fragmente und einer SL-Bandverletzung, welche per se von dorsal
versorgt werden muss. Die Operationstechnik erfolgte immer mit einer Retinaculumplastik und EPL-Sehnen Subkutanverlagerung. Allen Patienten wurde eine Implantatentfernung nach 6 Monaten empfohlen, da auch Spätrupturen der Sehnen vorkommen können. Von den 46 nachuntersuchten
Patienten zeigte sich ein sehr selektiertes Patientenkollektiv mit insgesamt 76% (35 Patienten) C Frakturen, über der Hälfte der Patienten hatte mit 52% (24 Patienten) eine C3 Fraktur, 30% (14 Patienten) SL-Bandverletzung und mehr als die Hälfte der Patienten (52%, 24 Patienten) hatte zudem eine relevante Begleitverletzung. Am Häufigsten davon die Frakturen am ipsilateralen Arm (9 Patienten, 20%), Karpusfrakturen (6 Patienten, 13%) und Polytrauma (6 Patienten, 13%). Das Patientengut zeigte sich selektiert aufgrund der sehr speziellen Indikationsstellung zur dorsalen Plattenosteosynthese, weshalb ein direkter Vergleich zur palmaren Plattenosteosynthese nicht möglich war. Die Studie dient dazu das klinische mittelfristige Outcome und die Komplikationsrate
nach dorsaler Plattenosteosynthese darzustellen. Keiner der Patienten erlitt eine Nervenverletzung, Revisionsoperation oder Sehnenschädigung. Interessant dabei zu erwähnen ist, dass sich nur 34 Patienten (74%) für eine Implantatentfernung entschieden, obwohl 6 Monate postoperativ allen Patienten eine Implantatentfernung empfohlen wurde. Trotzdem kam es zu keiner
Sehnenschädigung. 4 Patienten (14%) ohne relevante Begleitverletzung gaben einen seltenen Ruheschmerz an, der höchstens einmal alle 2 Wochen auftrat. 7 Patienten (15%) gaben einen leichten Belastungsschmerz bei mittelschwerer bis schwerer Belastung an und 4 Patienten (16%) entwickelten eine nach distaler Radiusfraktur typische Arthrose. Alle Patienten mit Arthrose erlitten initial eine C3 Fraktur. Die hälft davon zeigte einen Kellgran & Lawrence Score von 1. Der Bewegungsumfang des Handgelenks zeigte in allen Bewegungsgraden keinen signifikanten Unterschied zwischen der Fraktur- und Gegenseite, außer in der Palmarflexion. Trotzdem erreichte die Palmarflexion der Frakturseite durchschnittlich 93% der Gegenseite. Folgende Kraftmessungen wurden durchgeführt: Fingerkraft (Vigorimeter), Pinzettengriffkraft (Pinch-O-Meter) und Griffkraft (Dynamometer). In keiner Kraftmessung zeigte sich ein signifikanter Unterschied zwischen der Fraktur- und Gegenseite. Es wurden folgende radiologische Parameter erfasst: Palmare Kippwinkel, Radiopalmarwinkel, Ulnavarianz und Radiushöhe. Aufgrund der schweren Einstauchung und
dorsalen Abkippung präoperativ konnte postoperativ durchschnittlich nur eine annähernd normwertige Aufrichtung des palmaren Kippwinkels (von -3,1° auf 6,8°) und des Radiopalmarwinkels (von 19,3° auf 21,4°) erreicht werden, dennoch zeigte sich eine deutliche Aufrichtung des Gelenks. Die Funktionseinschränkung zeigte sehr gute Ergebnisse mit einem durchschnittlichen DASH Score von 5,5 ± 10. Der Gartland & Werley Score zeigte in 80% ein sehr gutes in 13% ein gutes, in nur 7% ein befriedigendes und bei keinem Patienten ein schlechtes Ergebnis. Die allgemeine Zufriedenheit wurde von den Patienten nach dem deutschen Schulnotensystem mit durchschnittlich
1,9 bewertet. Die dorsale Plattenosteosynthese bei distaler Radiusfraktur empfiehlt sich demnach bei CT gesicherter Indikationsstellung und richtiger Technik als modernes, alternatives Osteosyntheseverfahren.
When aiming at cell‐based therapies in osteoarthritis (OA), proinflammatory conditions mediated by cytokines such as IL‐1β need to be considered. In recent studies, the phytoalexin resveratrol (RSV) has exhibited potent anti‐inflammatory properties. However, long‐term effects on 3D cartilaginous constructs under inflammatory conditions with regard to tissue quality, especially extracellular matrix (ECM) composition, have remained unexplored. Therefore, we employed long‐term model cultures for cell‐based therapies in an in vitro OA environment and evaluated effects of RSV. Pellet constructs made from expanded porcine articular chondrocytes were cultured with either IL‐1β (1–10 ng/ml) or RSV (50 μM) alone, or a cotreatment with both agents. Treatments were applied for 14 days, either directly after pellet formation or after a preculture period of 7 days. Culture with IL‐1β (10 ng/ml) decreased pellet size and DNA amount and severely compromised glycosaminoglycan (GAG) and collagen content. Cotreatment with RSV distinctly counteracted the proinflammatory catabolism and led to partial rescue of the ECM composition in both culture systems, with especially strong effects on GAG. Marked MMP13 expression was detected in IL‐1β‐treated pellets, but none upon RSV cotreatment. Expression of collagen type I was increased upon IL‐1β treatment and still observed when adding RSV, whereas collagen type X, indicating hypertrophy, was detected exclusively in pellets treated with RSV alone. In conclusion, RSV can counteract IL‐1β‐mediated degradation and distinctly improve cartilaginous ECM deposition in 3D long‐term inflammatory cultures. Nevertheless, potential hypertrophic effects should be taken into account when considering RSV as cotreatment for articular cartilage repair techniques.
Background
The role of cement-augmented screw fixation for calcaneal fracture treatment remains unclear. Therefore, this study was performed to biomechanically analyze screw osteosynthesis by reinforcement with either a calcium phosphate (CP)-based or polymethylmethacrylate (PMMA)-based injectable bone cement.
Methods
A calcaneal fracture (Sanders type IIA) including a central cancellous bone defect was generated in 27 synthetic bones, and the specimens were assigned to 3 groups. The first group was fixed with four screws (3.5 mm and 6.5 mm), the second group with screws and CP-based cement (Graftys (R) QuickSet; Graftys, Aix-en-Provence, France), and the third group with screws and PMMA-based cement (Traumacem (TM) V+; DePuy Synthes, Warsaw, IN, USA). Biomechanical testing was conducted to analyze peak-to-peak displacement, total displacement, and stiffness in following a standardized protocol.
Results
The peak-to-peak displacement under a 200-N load was not significantly different among the groups; however, peak-to-peak displacement under a 600- and 1000-N load as well as total displacement exhibited better stability in PMMA-augmented screw osteosynthesis compared to screw fixation without augmentation. The stiffness of the construct was increased by both CP- and PMMA-based cements.
Conclusion
Addition of an injectable bone cement to screw osteosynthesis is able to increase fixation strength in a biomechanical calcaneal fracture model with synthetic bones. In such cases, PMMA-based cements are more effective than CP-based cements because of their inherently higher compressive strength. However, whether this high strength is required in the clinical setting for early weight-bearing remains controversial, and the non-degradable properties of PMMA might cause difficulties during subsequent interventions in younger patients.
The ability to differentiate into mesenchymal lineages, as well as immunomodulatory, anti-inflammatory, anti-apoptotic, and angiogenic properties give ASCs great therapeutic potential. Through their culture as multicellular, three-dimensional spheroids this potential can even be enhanced. Accordingly, 3D spheroids are not only promising candidates for the application in regenerative medicine and inflammatory disease therapy, but also for the use as building blocks in tissue engineering approaches. Due to the resemblance to physiological cell-cell and cell-matrix interactions, 3D spheroids gain higher similarity to real tissues, what makes them a valuable tool in the development of bioactive constructs equivalent to native tissues in terms of its cellular and extracellular structure. Especially, to overcome the still tremendous clinical need for adequate implants to repair soft tissue defects, 3D spheroids consisting of ASCs are a promising approach in adipose tissue engineering. Nevertheless, studies on the use of ASC-based spheroids as building blocks for fat tissue reconstruction have so far been very rare. In order to optimally exploit their therapeutic potential to further their use in regenerative medicine, including adipose tissue engineering approaches, a 3D spheroid model consisting of ASCs was characterized extensively in this work. This included not only the elucidation of the structural features, but also the differentiation capacity, gene expression, and secretory properties. In addition, the elucidation of underlying mechanisms contributing to the improved therapeutic efficiency was addressed.
Background: The soft tissue of the central pretibial area is difficult to reconstruct often requiring free tissue transfer. Especially medi- cally compromised patients are not ideal candidates for free tissue transfer and may benefit from expeditiously harvested local flaps with limited donor site morbidity. As muscle flaps are rare, pedi- cled flaps based on lateral perforators represent an alternative as the arc of rotation can often be limited to 90 °.
Material and Methods: A retrospective analysis of patient data was conducted to identify patients over the age of 60 years with comor- bidities that underwent pretibial soft tissue reconstruction with a single-pedicle perforator flap. Patient demographics, size and cause of the defect, flap dimension, arc of rotation and complications were recorded.
Results: Five patients with an average age of 71.4 years were in- cluded. The arc of rotation was 69 °, all flaps healed. There were two recurrences of osteomyelitis.
Conclusion: Lateral perforators originating from the anterior tib- ial artery or peroneal artery are adequate source vessels for single pedicled perforator flaps even in medically compromised patients. A perforator located proximal to the defect allows limiting the arcof rotation to less than 90 °, which increases the safety of the flap. Patients benefit from a simple procedure without a microvascular anastomosis and a donor site confined to one extremity
