The neurodegenerative disorder Alzheimer's disease (AD) is the cause of approximately 60% of the world's 35 million patients suffering from dementia. Current research focuses here are on association with other diseases such as diabetes type 2 (T2DM), possible genetic markers, specific signal transduction pathways within the brain and potential protein modification, because the pathogenesis and etiology of AD are still not fully understood. Specifically association of T2DM with AD came to the focus with the so-called "Rotterdam study" in 1999, indicating that T2DM doubles the risk of developing AD. In the meantime, it is known that the prevalence rate in patients with T2DM is 30%. Drugs commonly used in the treatment of T2DM such as peroxisome proliferator-activated receptors gamma (PPARγ) agonists show improvement of the cognitive abilities in patients with early stage of dementia, with potential therapeutically relevance. Therefore it is important not only to investigate a link between these diseases, but also to investigate the insulin signaling pathway in the brain of AD patients. In order to investigate this complex issue in more details and demonstrate additional links between T2DM and AD, the present study used several basic biological methods to clarify the question: "Is impaired insulin signaling pathway within the brain crucial for the development of AD?" from several points of view. The methods used in this work have been i) an analysis of single nucleotide (SNP) polymorphism of the insulin-degrading enzyme gene (IDE) in relation to risk of AD and / or of T2DM, ii) post-mortem histochemical studies of brain tissue of patients with only AD, with AD combined with T2DM and with only T2DM compared with an age-matched control group, and iii.) investigations of neurochemical pathways and gene/protein expression changes of a human cell culture as a consequences of amyloid β (Aβ) treatment. After analysis of the IDE SNP polymorphism in the selected VITA (Vienna Trans Danube Aging) cohort disease-specific effects were discovered. The upstream polymorphism (IDE2) was found to influence AD risk in a protective manner, while the downstream polymorphism (IDE7) modified the T2DM risk. Based on the SNP results, the presented study delineate the model that IDE promoter and 3‟ untranslated region/downstream variation can have different effects on IDE expression, maybe a relevant endophenotype with disorder-specific effects on AD and T2DM susceptibility. Furthermore, the human post-mortem studies could show that both AD as well as T2DM patients had a significantly lower density of the insulin receptor (IR) in the hippocampus, whereas a significantly increased density of inactive phosphorylated PPARγ has been found and this persisted even in patients with both diseases. Summarizing the histological study, it was possible to reveal common histological features of AD and T2DM, but no direct connection between the two diseases. Although AD is nowadays not only characterized by amyloid-containing plaque deposits and by the hyperphosphorylation of tau protein, the excessive Aβ42 presence in the brains of AD patients is still playing a key role. Up to date it is still not entirely clear which physical form of Aβ42 is responsible for the development of AD. The present work investigated, what impact has the state of aggregation of Aβ42 on genes and proteins of the insulin signaling pathway and the amyloid cascade. It could be shown that the oligomeric variant enhanced specifically the gene and protein expression of glycogen synthase kinase (GSK) 3β and also the enzyme activity was significantly increased, but has in turn strongly inhibited the IR gene and protein expression. Additionally, the effect of Aβ42 on monoamine oxidase B (MAO-B) was examined. An effect of both aggregated forms of Aβ42 had on enzyme activity was discovered. However, the fibrillar variants led to significantly increased activity of MAO-B while the oligomeric variants inhibited the enzyme activity. Several previous studies have demonstrated the involvement of increased MAO-B activity in AD, but the present work provides for the first time a direct link between the states of aggregation of Aβ42 to enzyme activity. Finally the results of the presented thesis can be summarized to following conclusion: Although AD and T2DM sharing some degrees of common features, still there is a lack of direct association, and therefore the diseases must be considered more independent rather than linked. But the impaired cerebral insulin signaling pathway seems to be another manifested hallmark of AD.
This thesis concerned the design and examination of a scaffold for tissue engineering applications. The template for the presented scaffold came from nature itself: the intercellular space in tissues that provides structure and support to the cells of the respective tissue, known as extracellular matrix (ECM). Fibres are a predominant characteristic feature of ECM, providing adhesion sites for cell-matrix interactions. In this dissertation a fibrous mesh was generated using the electrospinning technique to mimic the fibrous structure of the ECM. Two base polymers were explored: a biodegradable polyester, poly(D,L-lactide-co-glycolide); and a functional PEG-based star polymer, NCO-sP(EO-stat-PO). This topic was described in three major parts: the first part was materials based, concerning the chemical design and characterisation of the polymer scaffolds; the focus was then shifted to the cellular response to this fibrous scaffold; and finally the in vivo performance of the material was preliminarily assessed. The first steps towards an electrospun mesh started with adjusting the spinning parameters for the generation of homogeneous fibres. As reported in Chapter 3 a suitable setup configuration was on the one hand comprised of a spinning solution that consisted of 28.5 w/v% PLGA RG 504 and 6 w/v% NCO-sP(EO-stat-PO) in 450 µL acetone, 50 µL DMSO and 10 µL of an aqueous trifluoroacetic acid solution. On the other hand an ideal spinning behaviour was achieved at process parameters such as a flow rate of 0.5 mL/h, spinneret to collector distance of 12-16 cm and a voltage of 13 kV. The NCO-sP(EO-stat-PO) containing fibres proved to be highly hydrophilic as the functional additive was present on the fibre surface. Furthermore, the fibres featured a bulk degradation pattern as a consequence of the proportion of PLGA. Besides the morphologic similarity to ECM fibres, the functionality of the electrospun fibres is also decisive for a successful ECM mimicry. In Chapter 4, the passive as well as active functionality of the fibres was investigated. The fibres were required to be protein repellent to prevent an unspecific cell adhesion. This was proven as even 6.5 % sP(EO-stat-PO) in the PLGA fibres reduced any unspecific protein adsorption of bovine serum albumin and foetal calf serum to less than 1 %. However, avidin based proteins attached to the fibres. This adhesion process was avoided by an additional fibre surface treatment with glycidol. The active functionalisation of NCO-sP(EO-stat-PO)/PLGA fibres was investigated with two fluorescent dyes and biocytin. A threefold, chemically orthogonal, fibre modification was achieved with these dyes. The chapters about the chemical and mechanical properties laid the basis for the in vitro chapters where a specific fibre functionalisation with peptides was conducted to analyse the cell adhesion and biochemical expressions. Beginning with fibroblasts in Chapter 5 the focus was on the specific cell adhesion on the electrospun fibres. While NCO-sP(EO-stat-PO)/PLGA fibres without peptides did not allow any adhesion of fibroblasts, a fibre modification with GRGDS (an adhesion mediating peptide sequence) induced the adhesion and spreading of human dermal fibroblasts on the fibrous scaffolds. The control sequence GRGES that has no adhesion mediating qualities did not lead to any cell adhesion as observed on fibres without modifications. While the experiments of Chapter 5 were a proof-of-concept, in Chapter 6 a possible application in cartilage tissue engineering was examined. Therefore, primary human chondrocytes were seeded on fibrous scaffolds with various peptide sequences. Though the chondrocytes exhibited high viability on all scaffolds, an active interaction of cells and fibres was only found for the decorin derived sequence CGKLER. Live-cell-imaging revealed both cell attachment and migration within CGKLER-modified meshes. As chondrocytes undergo a de-differentiation towards a fibroblast-like phenotype, the chondrogenic re-differentiation on these scaffolds was investigated in a long term cell culture experiment of 28 days. Therefore, the glycosaminoglycan production was analysed as well as the mRNA expression of genes coding for collagen I and II, aggrecan and proteoglycan 4. In general only low amounts of the chondrogenic markers were measured, suggesting no chondrogenic differentiation. For conclusive evidence follow-up experiments are required that support or reject the findings. The success of an implant for tissue engineering relies not only on the response of the targeted cell type but also on the immune reaction caused by leukocytes. Hence, Chapter 7 dealt with primary human macrophages and their behaviour and phenotype on two-dimensional (2D) surfaces compared to three-dimensional (3D) fibrous substrates. It was found that the general non-adhesiveness of NCO-sP(EO-stat-PO) surfaces and fibres does not apply to macrophages. The cells aligned along the fibres on surfaces or resided in the pores of the meshes. On flat surfaces without 3D structure the macrophages showed a retarded adhesion kinetic accompanied with a high migratory activity indicating their search for a topographical feature to adhere to. Moreover, a detailed investigation of cell surface markers and chemokine signalling revealed that macrophages on 2D surfaces exhibited surface markers indicating a healing phenotype while the chemokine release suggested a pro-inflammatory phenotype. Interestingly, the opposite situation was found on 3D fibrous substrates with pro-inflammatory surface markers and pro-angiogenic cytokine release. As the immune response largely depends on cellular communication, it was concluded that the NCO-sP(EO-stat-PO)/PLGA fibres induce an adequate immune response with promising prospects to be used in a scaffold for tissue engineering. The final chapter of this thesis reports on a first in vivo study conducted with the presented electrospun fibres. Here, the fibres were combined with a polypropylene mesh for the treatment of diaphragmatic hernias in a rabbit model. Two scaffold series were described that differed in the overall surface morphology: while the fibres of Series A were incorporated into a thick gel of NCO-sP(EO-stat-PO), the scaffolds of Series B featured only a thin hydrogel layer so that the overall fibrous structure could be retained. After four months in vivo the treated defects of the diaphragm were significantly smaller and filled mainly with scar tissue. Thick granulomas occurred on scaffolds of Series A while the implants of Series B did not induce any granuloma formation. As a consequence of the generally positive outcome of this study, the constructs were enhanced with a drug release system in a follow-up project. The incorporated drug was the MMP-inhibitor Ilomastat which is intended to reduce the formation of scar tissue. In conclusion, the simple and straight forward fabrication, the threefold functionalisation possibility and general versatile applicability makes the meshes of NCO-sP(EO-stat-PO)/PLGA fibres a promising candidate to be applied in tissue engineering scaffolds in the future.
Die invasive Aspergillose ist eine schwerwiegende opportunistische systemische Infektion bei immunsupprimierten Patienten. In dieser Arbeit wurden verschiedene PCR-Verfahren und ein ELISA-Verfahren zum Antigennachweis in Hinblick auf Sensitivität, Sensibilität und positiver bzw. negativer Vorhersagewahrscheinlichkeit verglichen. Die konventionelle 18S-PCR ist ein panfungales Assay und wegen zahlreicher falsch-positiver Ergebnisse nicht geeignet zur Frühdiagnose. Die ITS-PCR hat sich aufgrund guter Spezifität als vielversprechend erwiesen, muss aber noch mit größeren Fallzahlen evaluiert werden. Der Antigennachweis (Platelia, Fa. Bio-Rad) weist eine hohe Sensitivität und negativen prädiktiven Wert auf. Als vielversprechend dürfte zukünftig eine Kombination aus PCR und Antigennachweisverfahren gelten.
Untersuchung der diagnostischen Wertigkeit eines standardisierten Algorithmus in der Differentialdiagnostik der Hyponatriämie, der diagnostischen Wertigkeit von Copeptin (Spaltprodukt von ADH) als ergänzender Diagnosemarker der Hyponatriämie und Vergleich mit der diagnostischen Wertigkeit bekannter Volumenparameter.
Reaktive Sauerstoffspezies (ROS, engl. reactive oxygen species) sind hochreaktive Biomoleküle, die in geringen Konzentrationen ubiquitär als Produkte des normalen zellulären Metabolismus entstehen. Zum Schutz vor irreversiblen oxidativen Schädigungen durch diese Moleküle, besitzt der Organismus antioxidative Enzyme und nicht-enzymatische Antioxidantien, die ROS neutralisieren. Eine pathophysiologische Zunahme der Generierung und Freisetzung von ROS und/oder verminderte zelluläre Abwehrmechanismen gegen diese können, in Folge einer gestörten Redox-Homöostase, zur Ausbildung des sog. „oxidativen Stresses“ führen, der unter anderem mit einer Reihe kardiovaskulärer Erkrankungen assoziiert ist. Das Steroidhormon Aldosteron (ALDO), das der Gruppe der Mineralocorticoide angehört, besitzt für den Wasser- und Elektrolythaushalt eine essentielle Bedeutung. Als Agonist bindet ALDO an Mineralocorticoidrezeptoren (MR) in der Niere und im Kolon und stimuliert unter physiologischen Bedingungen über die Aktivierung des MR die renale Rückresorption von Natriumionen und Wasser. Epidemiologische Studien sowie in vitro und in vivo Untersuchungen konnten einen kausalen Zusammenhang zwischen erhöhten ALDO-Serumwerten und/oder einer disproportional erhöhten MR-Aktivierung und einer gesteigerten ROS-Generierung im Herz-Kreislaufsystem belegen. Im Gegensatz zu ALDO besitzt das Sexualsteroid 17β-Estradiol (E2) protektive kardiovaskuläre Effekte. E2 fungiert über zwei intrazelluläre Estrogenrezeptor (ER)-Subtypen, ERα und ERβ, die nach ligandabhängiger Aktivierung als nukleäre Transkriptionsfaktoren die Expression spezifischer Zielgene regulieren. Neben ER-vermittelten vasodilatatorischen und antiinflammatorischen Wirkungen innerhalb der Blutgefäße und des Myokards besitzt E2 unter anderem auch antioxidative Eigenschaften und das Potential die zelluläre Redox-Homöostase günstig zu beeinflussen. Auf Grund dieser Befunde wurde die Hypothese aufgestellt, dass E2 über einen ER-vermittelten Mechanismus dem ALDO-induzierten oxidativen Stress in glatten Gefäßmuskelzellen der Ratte (PRSMC, für engl. primary rat smooth muscle cells) entgegenwirkt. Zusätzlich sollte durch die Supplementation des ERα-spezifischen Agonisten 16α-LE2 und des ERβ-spezifischen Agonisten 8β-VE2 zu ALDO-behandelten Zellen untersucht werden, ob die beiden ER-Subtypen redundante, spezifische oder gegensätzliche Effekte bezüglich der aufgestellten Hypothese, besitzen. Durch Färbungen hormonbehandelter PRSMC mit dem ROS-sensitiven Fluoreszenzfarbstoff Dihydroethidium (DHE) und anschließender Quantifizierung der DHE-Fluoreszenzintensität konnte bestätigt werden, dass der ALDO-induzierte oxidative Stress durch die Kosupplementation von E2, 16α-LE2 und 8β-VE2 über einen ERα- und ERβ-vermittelten Mechanismus signifikant reduziert werden kann. Des Weiteren konnte im Verlauf dieser Arbeit belegt werden, dass die intrazelluläre Konzentration des Reduktionsäquivalents Nicotinamid-Adenin-Dinukleotid-Phosphat in seiner reduzierten Form (NADPH) in ALDO+E2-, ALDO+16α-LE2- und ALDO+8β-VE2-behandelten PRSMC im Vergleich zu nur mit ALDO-behandelten Zellen signifikant erhöht ist. Ferner konnte in Übereinstimmung mit diesen Ergebnissen gezeigt werden, dass die Kosupplementation von NADPH zu ALDO-behandelten Zellen, die DHE Oxidation in diesen Zellen signifikant unterbindet. Mittels Western Blot Analysen und Enzymaktivitätsassays hormonbehandelter PRSMC ließ sich schließlich nachweisen, dass ALDO die Enzymaktivität und Proteinexpression des NADPH generierenden Enzyms Glukose-6-phosphat Dehydrogenase (G6PDH) supprimiert. Weiter konnte innerhalb dieser Arbeit erstmalig bestätigt werden, dass E2 die Suppression der G6PDH-Aktivität durch ALDO unterbindet und die Enzymaktivität dieses Proteins wieder auf den basalen Wert anhebt. Zusammenfassend konnte in dieser Arbeit gezeigt werden, dass E2 der ALDO-induzierten lokalen ROS-Generierung in PRSMC ER-vermittelt entgegenwirkt. Der zugrunde liegende Mechanismus dieses Effekts basiert auf einer Aufrechterhaltung der G6PDH-Enzymaktivität, wodurch die Bioverfügbarkeit des Reduktionsäquivalents NADPH, das eine Schlüsselrolle im zellulären antioxidativen System spielt, gewährleistet wird.
Die Bedeutung der Expression des Tumornekrosefaktor-alpha bei Patienten mit kolorektalem Karzinom

Für Tumorprogression müssen entartete Zellen Wege finden, die immunologische Abwehr und die Apoptose zu umgehen. Tumorzellen haben dafür verschiedene, sogenannte Tumor-Escape-Mechanismen entwickelt. Gegenstand dieser Arbeit war in diesem Zusammenhang die Untersuchung des TNF-α-TNF-R1-Systems. Eine signifikante erhöhte TNF-α Protein- und Genexpression konnte in Gewebeproben kolorektaler Karzinome nachgewiesen werden, wobei eine mäßig starke Korrelation beider Analysemethoden ersichtlich war (т = 0.794). Sowohl die immunhistochemische Analyse als auch die Genexpression durch RT-PCR konnten mit Tumorprogression assoziiert werden. Mit erhöhter Expression des Apoptose-induzierenden Zytokins TNF-α durch Tumorzellen konnte darüber hinaus ein signifikant schlechteres Gesamtüberleben der Patienten mit KRK beobachtet werden. In unmittelbarer Umgebung TNF-α exprimierender Tumorzellen wurden zahlreiche TNF-R1+/CD8+ Zellen analysiert, die als Hinweis auf Apoptose in TILs angesehen werden können. Dieser Weg könnte als Tumor-Escape-Mechanismus verstanden werden. Die Verwendung potentieller TNF-α Biologicals (z.B. Etanercept, Infliximab) bleibt jedoch unter dem Aspekt einer geringfügig erhöhten Lymphominzidenz gegenüber der Normalbevölkerung auch als kritisch zu bewerten. Der Einsatz von Anti-TNF-α-Therapien stellt jedoch eine vielversprechende Option bei Patienten mit metastasiertem KRK und Tumorrezidiv dar. Zusammenfassend liefern die Ergebnisse dieser Arbeit zusätzlichen Einblick in die Regulationsmechanismen, die verantwortlich für die Immunsuppression durch kolorektale Karzinome sein können. Diese basiswissenschaftlichen Erkenntnisse stellen eine mögliche Grundlage neuer Behandlungsmöglichkeiten kolorektaler Karzinomen dar, die weiter erforscht werden sollten.
Ein Zeitgewinn in der frühen klinischen Versorgung polytraumatisierter Patienten ist nach wie vor eines der vorrangigen Ziele moderner Schockraumalgorithmen. Insbesondere Patienten, die unmittelbar im Anschluss an die primäre klinische Versorgung operativ versorgt werden müssen, könnten von einer verkürzten Zeit bis zum Beginn des operativen Eingriffes profitieren. Die Primärdiagnostik mittels Ganzkörper-MSCT hat in den letzten Jahren zunehmend an Bedeutung in der Schockraumversorgung polytraumatisierter Patienten gewonnen. Ihr entscheidender Vorteil könnte insbesondere die Zeitersparnis durch den schnelleren diagnostischen Arbeitsablauf im Rahmen eines MSCT-gestützten Schockraumalgorithmus sein. Bisherige Studien berichteten über deutliche Zeitgewinne im Vergleich zur konventionellen Röntgendiagnostik. Es wird aber weiterhin diskutiert, welches Verfahren als „golden standard“ gilt, insbesondere welches Verfahren das klinische Outcome polytraumatisierter Patienten verbessern kann. In der vorliegenden Studie wurde daher ein Vergleich zwischen einem konventionellen Schockraumalgorithmus mit konventioneller Röntgendiagnostik, Sonographie und organfokussiertem CT sowie einem modernen Schockraumalgorithmus mit fester Integration der Ganzkörper-MSCT gezogen. Es wurde die Hypothese untersucht, ob die MSCT eine signifikante Zeitersparnis in der Schockraumversorgung polytraumatisierter Patienten bis zum Beginn dringlicher operativer Eingriffe bringt und ob dieser Zeitgewinn eine Verbesserung des klinischen Outcome bedeutet. In der vorliegenden retrospektiven Studie wurden im Zeitraum von 2001-2007 zwei Gruppen von Schockraumpatienten erfasst: In Gruppe I wurden 155 Patienten von 2001-2003 eingeschlossen, die nach dem konventionellen Schockraumalgorithmus behandelt wurden. In Gruppe II wurden von 2004-2007 163 polytraumatisierte Patienten eingeschlossen, die primär im Rahmen eines MSCT-gestützten Schockraumalgorithmus versorgt wurden. Bei allen Patienten wurden die tOR als Zeitintervall zwischen Beginn der Schockraumversorgung und Beginn des operativen Eingriffes sowie demographische und klinische Daten erfasst. Das klinische Outcome wurde mittels der Intensivliegezeit, der Beatmungsdauer und der Mortalität untersucht. Bei dem Vergleich der Gruppen wurde als nicht-parametrischer Test der Man-Whitney-Rank Sum Test verwendet, mit p<0.05 als Signifikanzniveau. Die tOR in Gruppe I war 120 Minuten (Interquartilabstand 90-150 Minuten), während sie in Gruppe II 105 Minuten (85-133 Minuten) betrug (p<0.05). In beiden Gruppen starben insgesamt vierzehn Patienten, davon fünf in den ersten vierundzwanzig Stunden. Die Intensivliegezeit betrug in Gruppe I fünf Tage (Interquartilabstand 1-14), in Gruppe II acht Tage (Interquartilabstand 2-19), p=0,157. In Gruppe I wurden die Patienten drei Tage (Interquartilabstand 1-12) maschinell beatmet, in Gruppe II fünf Tage (Interquartilabstand 1-15). Zusammenfassend kann gesagt werden, dass die Einführung der MSCT als primäre Schockraumdiagnostik die tOR, also das Zeitintervall bis zum Beginn dringlicher operativer Eingriffe verkürzt. Diese Aussage passt zur vorherrschenden Meinung in der aktuellen Literatur. Das Outcome konnte im Gegensatz zu anderen Studien nicht verbessert werden. Es ist davon auszugehen, dass einen Hauptgrund hierfür ein signifikant höherer ISS-Wert in der Gruppe II darstellt. Für eine definitive Aussage über eine mögliche Verbesserung des klinischen Outcome durch die MSCT als Primärdiagnostik im Schockraum sind weitere Untersuchungen an einem größeren Patientenkollektiv über einen längeren Zeitraum erforderlich.
Das akute Nierenversagen aufgrund ischämischer oder toxischer Nierenschädigung ist eine häufig auftretende Erkrankung mit einer hohen Mortalität. Immer noch sind die genauen Abläufe auf Zellebene nicht vollständig geklärt. Diese Arbeit befasst sich mit der Rolle der induzierbaren Hämoxygenase (HO-1) und ihrer Wechselwirkung mit der endothelialen Stickoxidsynthase (eNOS) im akuten ischämischen Nierenversagen. Dazu wurde im Rattenmodell mittels 45-minütigen Clamping beider Aa. renales ein ischämisches ANV induziert. Während eine Gruppe dem Clamping ohne weitere Intervention unterzogen wurde, ist einer weiteren Gruppe Zinn(VI)-Mesoporphyrin (SnMP), ein Inhibitor der HO-1, intraperitoneal appliziert worden. Eine dritte Gruppe mit Schein-Operation wurde als unbehandelte Kontrollgruppe parallel operiert und untersucht. Zur Bestimmung von renaler Filtrations- und Sekretionsfunktion wurden die Inulin-Clearance und die PAH-Clearance gemessen. Die Expression der untersuchten Enzyme aus HO/CO- und NOS/NO-System wurde mittels Westernblot quantitativ bestimmt. Der postinterventionelle Versuchszeitraum erstreckte sich über 14 Tage. In der unbehandelten Clamping-Gruppe zeigte sich ein Abfall von Inulin- und PAH-Clearance in der frühen postischämischen Phase, gefolgt von einer raschen Erholung und stetiger Verbesserung der Exkretionsfunktion im Verlauf des 14-tägigen Versuchs-intervalls. Die HO-1 zeigte einen Tag nach der Ischämie eine starke Überexpression, ebenso die im Hämkatabolismus wichtige Biliverdinreduktase (BVR). Die eNOS reagierte mit einer Hochregulierung, die sich in der Medulla langsamer entwickelte als in Kortex. Der gemeinsame Rezeptor der Signalstoffe CO und NO, die lösliche Guanylatzyklase (sGC), zeigte eine bedarfsgesteuerte leichte Hochregulation, möglicherweise ein Zeichen der endothelialen Dysfunktion mit zunächst nicht ausreichender NO-Bildung durch die eNOS. Die Clamping + SnMP-Gruppe erlitt eine stärkere Funktionseinschränkung nach der Intervention, was sich mit der Hemmung der HO-1 erklärt, regenerierte sich anschließend rasch wieder und hatte im Versuchsverlauf vergleichbare Funktionswerte wie die Clamping-Gruppe. Die HO-1 wurde ebenso akut hochreguliert, hielt aber die starken Überexpressionswerte während des Versuchszeitraums länger aufrecht als die Clamping-Gruppe. Diese Beobachtung lässt sich mit der Eigenschaft des SnMP erklären, dass es die Aktivität der HO-1 hemmt, ihre Expression aber steigert. Die Hochregulation der BVR war in dieser Gruppe geringer ausgeprägt. Die eNOS wurde in den mit SnMP behandelten Tieren weit schwächer exprimiert. Die sGC zeigte ähnliche Werte wie in der Clamping-Gruppe, wenn auch weniger stark reguliert. Die Beobachtung, dass sich eine Hemmung der HO-1 nicht nur auf die Enzyme ihres eigenen Signalweges und des Hämabbaus, der sGC und der BVR, auswirkt, sondern auch auf die der eNOS, bestätigt, dass das HO/CO- und das NOS/NO-System eng mit einander verknüpft sind. Dass dies nicht nur unter physiologischen Bedingungen, sondern insbesondere in der pathologischen Situation einer renalen Ischämie zum Tragen kommt, eröffnet Perspektiven, dass die dargestellten Ergebnisse dazu dienen könnten, neue Ansätze zu finden, um die endotheliale Dysfunktion im ANV kurativ zu beeinflussen.
Eine Schädigung des Endothels ist früh nachweisbar in der Pathogenese kardiovaskulärer Erkrankungen. Beim Diabetes mellitus führt die Entkopplung der endothelialen Stickstoffmonoxid-Synthase (eNOS) durch Bildung von Superoxidanionen (O2-) anstatt von Stickstoffmonoxid (NO) zu einer gesteigerten Produktion an reaktiven Sauerstoffradikalen (ROS) und zu einer Schädigung des Endothels. Bei der Endothelregeneration spielen die kürzlich entdeckten endothelialen Progenitorzellen (EPCs) eine entscheidende Rolle. Für deren Mobilisierung und volle Funktionalität ist die eNOS von essentieller Bedeutung. In der vorliegenden Arbeit wurde die Hypothese untersucht, daß die beim Diabetes mellitus bekannte Entkopplung der eNOS auch eine wichtige Rolle bei der verminderten Mobilisierung und Dysfunktion von EPCs spielen könnte. Bei Patienten mit Typ-II Diabetes waren die EPC-Spiegel im Blut deutlich vermindert, die EPCs von diabetischen Patienten produzierten mehr O2- und ihre Funktion war im Vergleich zu den EPCs von Kontrollen eingeschränkt. Die gestörte Funktion der EPCs ließ sich durch eine Blockade der NOS mit NG-nitro-L-Arginin (L-NNA) zu einem großen Teil wiederherstellen. Gleichzeitig war dies auch mit einer verminderten O2--Produktion verbunden. In kultivierten EPCs führte die Inkubation mit Glukose zu einer vermehrten O2-- Produktion und einer verminderten Migrationsfähigkeit. Die Proteinkinase C scheint hierbei mechanistisch über eine Aktivierung der NADPH-Oxidase (NOX) von Bedeutung zu sein. Die durch Glukose hervorgerufene gesteigerte O2--Generierung resultiert in verminderten intrazellulären Tetrahydrobiopterin (BH4) -Spiegeln, dem wahrscheinlich entscheidenden pathophysiologischen Mechanismus bei der eNOS-Entkopplung. Nach exogener Zufuhr von BH4 kam es zu einer signifikanten Funktionsverbesserung der EPCs und einer deutlich verminderten O2--Produktion. Im Tiermodell des Diabetes wurden EPC-mobilisierende Mechanismen untersucht. Bei Ratten wurde durch Streptozotozininjektion ein Typ-I-Diabetes hervorgerufen. Bei diesen Tieren konnten ebenso wie bei den diabetischen Patienten verminderte EPC-Spiegel nachgewiesen werden. Ursache hierfür könnte eine Entkopplung der eNOS im Knochenmark sein. Hier zeigte sich eine gesteigerte O2--Produktion, welche durch eine NOS-Blockade mittels L-NNA teilweise reversibel war. Wahrscheinlich sind die auf die Entkopplung der eNOS zurückzuführende verminderte EPC-Mobilisierung und -Funktion mitbestimmende Faktoren in der Pathogenese von vaskulären Komplikationen beim Diabetes mellitus.
