Background: The use of assisted reproductive techniques (ART) for treatment of infertility is increasing rapidly worldwide. However, various health effects have been reported including a higher risk of congenital malformations. Therefore, we assessed the risk of anorectal malformations (ARM) after in-vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI).
Methods: Data of the German Network for Congenital Uro-REctal malformations (CURE-Net) were compared to nationwide data of the German IVF register and the Federal Statistical Office (DESTATIS). Odds ratios (95% confidence intervals) were determined to quantify associations using multivariable logistic regression accounting for potential confounding or interaction by plurality of births.
Results: In total, 295 ARM patients born between 1997 and 2011 in Germany, who were recruited through participating pediatric surgeries from all over Germany and the German self-help organisation SoMA, were included. Controls were all German live-births (n = 10,069,986) born between 1997 and 2010. Overall, 30 cases (10%) and 129,982 controls (1%) were born after IVF or ICSI, which translates to an odds ratio (95% confidence interval) of 8.7 (5.9-12.6) between ART and ARM in bivariate analyses. Separate analyses showed a significantly increased risk for ARM after IVF (OR, 10.9; 95% CI, 6.2-19.0; P < 0.0001) as well as after ICSI (OR, 7.5; 95% CI, 4.6-12.2; P < 0.0001). Furthermore, separate analyses of patients with isolated ARM, ARM with associated anomalies and those with a VATER/VACTERL association showed strong associations with ART (ORs 4.9, 11.9 and 7.9, respectively). After stratification for plurality of birth, the corresponding odds ratios (95% confidence intervals) were 7.7 (4.6-12.7) for singletons and 4.9 (2.4-10.1) for multiple births. Conclusions: There is a strongly increased risk for ARM among children born after ART. Elevations of risk were seen after both IVF and ICSI. Further, separate analyses of patients with isolated ARM, ARM with associated anomalies and those with a VATER/VACTERL association showed increased risks in each group. An increased risk of ARM was also seen among both singletons and multiple births.
Einleitung: In den letzten Jahren hat sich das intraoperative, elektrophysiologische Neuromonitoring des Nervus recurrens in der Schilddrüsenchirurgie mehr und mehr etabliert und durchgesetzt. Patienten und Methode: In einer prospektiven Studie wurden 79 Patienten (61 weiblich, 19 männlich) an der Schilddrüse operiert. Dabei haben wir das System Nadelelektrode, bei dem die Elektrode durch das Ligamentum crycothyroideum in den Muskulus vocalis gestochen wird (Neurosign 100, Fa. Inomed) mit dem System Tubuselektrode, bei dem die Elektroden dem Stimmband anliegen (NIM-Response 2.0, Fa. Medtronic), verglichen. Es wurde der Nervus vagus, der Nervus laryngeus recurrens und der Nervus laryngeus superior identifiziert und ein EMG-Signal für jeden Nerv abgeleitet. Anschließend wurden hieraus Amplituden und Latenzzeizen der jeweiligen Ableitung bestimmt. Ergebnisse: Für den linken Nervus vagus ergab sich eine mittlere konstante Latenzzeit im Bereich von ca. 6,3 ms, die damit ca. 2,5 ms länger als die ebenfalls konstante Latenzzeit des rechten Nervus vagus (ungefähr 3,8 ms) war. Beim Nervus laryngeus recurrence wurde auf beiden Seiten eine konstante durchschnittliche Latenzzeit von ca. 2,2 -2,5 ms gemessen. Die Darstellung des Nervus laryngeus superior gestaltete sich technisch schwieriger und gelang nur in 37% der Fälle. Bei den identifizierten Nerven lagen im Mittel sowohl für den linken als auch für den rechten Nervus laryngeus superior mittlere konstante Latenzzeiten zwischen 1,8 ms und 2,0 ms vor. Zwischen beiden Neuromonitoringsystemen („Tubuselektrode“ vs. „Nadelelektrode“) fanden sich bezüglich der Latenzzeiten keine Unterschiede. Die Bestimmung der Amplituden ergab für alle drei Nerven bei den absoluten Messwerten ein inhomogenes Bild. Es zeigte sich allerdings, dass mit der Nadelelektrode ein im Mittel ca. drei- bis fünffach stärkeres Signal als mit der Tubuselektrode abgeleitet werden konnte. Schlussfolgerung: Aus den Ergebnissen kann die Forderung abgeleitet werden, dass das Neuromonitoring in Zukunft nicht nur zur Identifikation des Nervus laryngeus recurrence genutzt werden sollte, sondern auch zur Bestimmung von Amplitude und Latenzzeit, da insbesondere bei Amplitudenveränderungen mögliche Nervenschädigungen frühzeitig erkannt werden könnten. Da mit der Nadelelektrode grundsätzlich höhere Signalstärken zu erfassen waren, ist diese zu bevorzugen. Das Neuromonitoring hat sich als sinnvolles technisches Hilfsmittel bei Schilddrüseneingriffen herausgestellt, kann helfen die Rekurrenspareserate zu senken, erhöht die Sicherheit in der Ausbildung von jungen Operateuren und bietet die Möglichkeit der heute geforderten Dokumentation über die Nervendarstellung.
Die Narbenhernie stellt eine häufige Komplikation nach Laparotomien dar. Die Therapie der Narbenhernie erfolgt mittels chirurgischer Netzimplantation. Dieses Verfahren erfordert detaillierte anatomische Kenntnisse. Dem ethischen Imperativ folgend, wurde ein kosten-effizientes Modell entwickelt, welches den humanen Situs imitiert und an dem sich eine retromuskuläre Netzimplantation durchführen lässt. Das High-Fidelity-Modell besteht zum Hauptteil aus 2-Komponenten-Silikon. Das Modell wurde entwickelt und im Rahmen dieser Studie validiert. Zur Ermittlung der Testpersonenanzahl wurde die Methodik des sequentiellen Dreieckstests genutzt. Nachdem 6 Anfänger (PJ-Studierende) und 6 Könner (Fachärzte für Viszeralchirurgie) jeweils ein Modell operiert hatten, wurde die Kontent-, die Konstrukt- und die Kriterienvalidität untersucht. Anschließend wurde das Modell und die Operationsdurchführung mit drei Methoden untersucht. Zum einen füllten die Teilnehmenden einen Fragebogen bezüglich der Realitätsnahe des Modells direkt nach der Operation aus. Außerdem bewerteten drei verblindete Bewerter die Operationen anhand des Competency assessment tool (CAT), welcher eine modifizierte Version des Fragebogens nach Miskovic darstellt, nach den folgenden Subskalen: „Instrumentengebrauch“, „Umgang mit dem Gewebe“, „Mängel und Fehler“, „Qualität des Endprodukts“. Zuletzt wurden die operierten Modelle bezüglicher der „Endergebnisse“ autopsiert und bewertet.
Die Ergebnisse zeigen, dass am SUBsON-Modell eine Narbenhernienoperation mit Netzimplantation authentisch durchgeführt werden kann. Die Testpersonen bewerteten das Modell als realitätsnah. Die Reliabilität war in allen Kategorien gut bis exzellent. Die Könner waren in allen Subskalen des CATs den Anfängern überlegen. Bei Betrachtung der Kriterienvalidität zeigte sich ein paradoxer Effekt: Bei der Präparation des Fatty Triangles erbrachten die Anfänger eine signifikant (p< 0,05) höhere Leistung als die Könner. Mögliche Erklärungen dafür sind mannigfaltig.
Die Leistungsunterschiede zwischen Anfängern und Könnern bestätigen die Konstruktvalidität von Modell und Fragebogen sowie die Realitätsnähe des Modells. In dieser Studie konnten Defizite vor allem unter Könnern bezüglich anatomischer Kenntnisse bei der Präparation des Fatty Triangles aufgezeigt werden. Das Modell kann zukünftig genutzt werden, um die Netzimplantation und die Präparation des Fatty Triangles zu üben als auch um die chirurgischen Leistungen zu evaluieren.
Es geht um die Colonanstomosenheilung am Tiermodell der Wistarratte. Im Tierexperiment wurden Handnahtanastomosen am Colon angefertigt und zu bestimmten Zeitpunkten physikalisch und zytokinetisch nachuntersucht. Ziel war unter anderem der Nachweis eines charakteristischen Zytokinmusters (TGF beta, IL-10, RANTES)um die Wundheilung am Colon nach Anastomosierung besser zu verstehen.
Die qualitative und quantitative Narbenqualitätsmessungen im Verlauf der Wundheilung ist von großer Bedeutung, da durch sie Einflussgrößen, die mit hoher Wahrscheinlichkeit zu pathologischen Narbenformationen führen, frühzeitig erkannt werden könnten und Rückschlüsse auf die jeweils angewandten Techniken und Materialien zulassen würden. Dass für die Analyse der Heilung von Schnitt- und Defektwunden objektive Messungen unabdingbar sind, sollte am Beispiel des Göttinger Miniaturschweins gezeigt werden. Hierzu wurden verschiedene Naht-/Transplantationsmaterialien und Naht-/Transplantationsmethoden der Wundbehandlung angewandt, deren Heilungsergebnisse im Verlauf durch eine quantitative Beurteilung vergleichbar wurden. Außer auf der tensiometrischen und histologischen Begutachtung lag das Hauptaugenmerk auf der quantitativen Erfassung der Oberflächenstrukturen. Mit einem topometrischen System (OPTOCAT-3D) wird für jeden Kamera-Bildpunkt die 3D-Koordinate des zugehörigen Objektpunktes gemessen. Somit können mit einer Teilansicht mehrere tausend Messpunkte erfasst und ausgewertet werden. Diese ermöglicht die kontaktfreie Volumenmessung und die Darstellung von Höhen- und Flächendifferenzen bei Narbenformationen oder eingeheilten Hauttransplantaten in Gegenüberstellung zur gesunden Haut. Die in dieser Studie erstmals angewandte Methode der kontaktfreien topometrischen Erfassung und Beurteilung der Narbenqualität in Kombination mit der bewährten Tensiometrie und der histopathologischen Begutachtung machte es möglich, Daten zu erhalten, über die eine objektive Beurteilung der Narbenqualität erstellt werden kann.
EINLEITUNG: Zur Behandlung der pAVK existieren verschiedene Behandlungsansätze. In dieser Studie soll die konservative intravenöse Therapieform, bestehend aus den Prostaglandinen Ilomedin® und Prostavasin® sowie dem Medikament Dusodril®, dargestellt werden. Die Schwerpunkte liegen auf der Betrachtung des Beinerhalts und des Überlebens sowie einem Vergleich zur Bypasstherapie und der Betrachtung des Kosten-Erlös-Verhältnisses. METHODEN: Anhand der SAP-Abrechnungsdatei, einer Datenbank und der Stationsbücher wurden alle Patienten der gefäßchirurgischen Station erfasst, die zwischen den Jahren 1993 und 2007 an der Universitätsklinik Würzburg auf Grund einer pAVK konservativ behandelt wurden. Zusätzlich wurden die entsprechenden Hausärzte anhand einer Follow-Up Studie zu dem weiteren Krankheitsverlauf der Patienten befragt. ERGEBNISSE: Von den insgesamt 94 Patienten litten 80 (85,1 %) an einer kritischen Ischämie. Je 40 (42,5 %) Patienten erhielten Ilomedin® und Prostavasin®, 14 (15 %) wurden mit dem Medikament Dusodril® therapiert. Die mittlere Therapiedauer lag bei 9,1 Tagen. Insgesamt mussten 18 Majoramputationen (19,1 %) durchgeführt werden, wobei besonders Patienten im Stadium 4 (88,8 %) betroffen waren (p=0,0024). Der 5-Jahres-Beinerhalt der Gesamtkollektivs lag bei 77,6 %. Schon im ersten Jahr nach Therapieende verstarben 22 % der Patienten, wobei sich sowohl die Herzinsuffizient (p=0,004), als auch eine KHK (p=0,008) als prognostisch ungünstig erwiesen. Ein Zusammenhang zwischen den pAVK-Stadien und den Überlebenschancen der jeweiligen Patienten konnte nicht festgestellt werden. Die mittlere Lebenserwartung des Kollektivs lag bei 3,5 Jahren. Sowohl in der Gruppe mit, als auch in der Gruppe ohne Revaskularisationsmöglichkeit war der Wert für den 5-Jahres- Beinerhalt für die Patienten ohne Bypass günstiger (94 % bzw. 75 %), als für die Patienten, die sich einer Bypassoperation unterzogen hatten (70 % bzw. 60 %). Der Kosten-Erlös-Vergleich zeigte eine Unterdeckung in allen Stadien der pAVK, welche im Stadium 2 besonders gravierend war (3751,38 € Kosten pro Patient vs. 1532 € Erlös [F65D] nach DRG-Prinzip). 80,7 % dieser Kosten sind auf den stationären Aufenthalt zurückzuführen. SCHLUSSFOLGERUNG: Die konservative intravenöse Therapieform der pAVK hat, auch bei einer deutlich kürzeren Therapiedauer als empfohlen, einen anhaltend amputationsvermeidenden und präventiven Effekt in Fällen. Bei Patienten mit einer Revaskularisationsmögichkeit scheint sie zudem eine Alternative zur Bypasstherapie darzustellen. Das Kosten-Erlös-Verhältnis ist allerdings, auf Grund der hohen anfallenden Kosten des stationären Aufenthalts, als ungünstig einzustufen.
In der vorliegenden Arbeit wurde bei 30 Patienten mit einer Arthroskopie des Handgelenkes in Plexusanästhesie der analgetische Effekt von 5ml 0,5%igem intraartikulär appliziertem Bupivacain untersucht. Die Eingriffe fanden zwischen 1995 bis 1998 an der Chirurgischen Klinik der Universität Würzburg statt. Einer Gruppe von 12 Patienten wurde intraoperativ intraartikulär Bupivacain injiziert. 18 Patienten dienten als Kontrollgruppe ohne Schmerzmittelgabe während des Eingriffs. Postoperativ wurde die Analgesie in den ersten 12 Stunden und den darauffolgenden vier Tagen mittels Verbaler Ratingskala (VRS) und Visueller Analogskala (VAS) bestimmt. Zusätzlich wurde der postoperative Analgetikabedarf festgehalten. Bei der Auswertung der VRS zeigte sich in den ersten drei postoperativen Stunden ein signifikant niedrigere Schmerzintensität der Verum- gegenüber der Kontrollgruppe. Bei der Auswertung der VAS ließ sich in der Verumgruppe ein nicht signifikant niedrigeres Schmerzniveau nachweisen. Ein indirektes Maß für den Schmerzverlauf und die Schmerzintensität der Patienten ist der postoperative Analgetikabedarf. In der vorliegenden Studie waren nur bei der Hälfte des gesamten Patientenkollektivs Analgetika notwendig. Das gesamte Patientenkollektiv verspürte postoperativ nur „mäßig starke“ Schmerzen. Das postoperative Schmerzniveau bewegte sich somit in der unteren Hälfte der maximal anzugebenden Schmerzintensität. Der Einnahmezeitpunkt des ersten Schmerzmittels nach der Untersuchung war bei der Verumgruppe später als bei der Kontrollgruppe (438min +/- 173min vs. 380min +/- 210min), aber nicht signifikant. Wurden die Operationskategorien unabhängig von der intraartikulären Schmerzmittelapplikation in „kleine Eingriffe“ (reine Arthroskopie, Arthroskopie mit Biopsieentnahme, Arthroskopie mit Entfernung eines freien Gelenkkörpers) und „große Eingriffe“ (Arthroskopie mit Biopsieentnahme und Shaving, Arthroskopie mit Fensterung des DUC, arthroskopische Resektion des distalen Ulnakopfes (Wafer-Procedure)) unterteilt, zeigte sich, dass das Ausmaß der Beschwerden in den ersten 12 Stunden postoperativ in der Gruppe „große Eingriffe“ etwas höher lag als in der Gruppe „kleine Eingriffe“. In der zweiten Stunde nach Arthroskopieende war der Unterschied signifikant. Obwohl das Schmerzniveau insgesamt beim vorliegendem Patientenkollektiv niedrig war, fand sich bei den Patienten mit intraartikulärer Gabe eines Lokalanästhetikums (Bupivacain) in den ersten drei Stunden nach Operationsende eine signifikante Schmerzminderung, ein späterer Zeitpunkt der Analgetikaeinnahme und ein geringerer postoperativer Analgetikabedarf gegenüber einer Kontrollgruppe ohne diese Behandlung.
Es wird vermutet, dass das ösophageale Adenokarzinom (EAC) durch gastroosophagealen Reflux auf dem Boden des Barrett-Ösophagus (BE) entsteht. Bei der Tumorprogression könnten Matrix-Metalloproteasen eine wichtige Rolle spielen.
Die Expression von MMP-1 und MMP-13 wurde im Ösophaguskarziom (n=41 EAC mit BE, n=19 EAC ohne BE, n=10 Plattenepithelkarzinom, ESCC) sowie im nicht-dysplastischen BE (n=18) untersucht. Die Koexpression von MMP-1 und Cdx-2 (intestinale Metaplasie) und die Koexpression von MMP-1 und Ki-67 (Proliferation) wurde mittels Immunhistochemie und auf mRNA-Ebene untersucht. Die Ergebnisse wurde mit klinisch-pathologischen Eigenschaften korreliert.
Im gesunden Plattenepithel wurde weder MMP-1 noch MMP-13 exprimiert. In allen EAC ohne BE wurde MMP-1 exprimiert (100%). Im EAC mit BE, war in 95% MMP-1 im EAC nachweisbar. Die Expression von MMP-1 im BE ohne IN lag bei 56%. Das ESCC exprimierte in 60% MMP-1. Bei der quantitativen Analyse zeigten sich 48% MMP-1 positive Zellen im EAC mit BE und 35% im angrenzendem BE (p<0,05). Mit 44% MMP-1 positiver Zellen im EAC ohne BE, lag die Expression signifikant über der im BE mit EAC (p<0,05). Im ESCC (32% MMP-1 positiv) lag eine im Vergleich zu allen EACs signifikant geringere Expression vor. Im BE ohne IN waren 4% der Zellen MMP-1 positiv. Die RT-PCR bestätigte die Ergebnisse der IHC auf mRNA-Ebene. Eine Präparate waren negativ für MMP-13. Die Untersuchung der Koexpression von MMP-1 in Ki-67 positiven Zellen zeigte eine starke direkte Korrelation (r=0,943 für BE und r= 0,811 für EAC). Eine hohe MMP-1 Expression war mit einem positiven Lymphknotenstatus assoziiert aber nicht mit einem schlechterem Überleben (p=0,307). Die Ergebnisse zeigen, dass MMP-1 eine wichtige Rolle bei der Invasion und Metastasierung des Barrett assoziierten EAC spielen könnte. Die Assoziation eines positiven Lymphknotenstatus mit hoher MMP-1-Expression spricht dafür, dass MMP-1 ein wichtiger Faktor bei der malignen Progression sein könnte.
Fibroblast growth factor-inducible 14 (Fn14) is a member of the tumor necrosis factor (TNF) receptor superfamily (TNFRSF) and is activated by its ligand TNF-like weak inducer of apoptosis (TWEAK). The latter occurs as a homotrimeric molecule in a soluble and a membrane-bound form. Soluble TWEAK (sTWEAK) activates the weakly inflammatory alternative NF-κB pathway and sensitizes for TNF-induced cell death while membrane TWEAK (memTWEAK) triggers additionally robust activation of the classical NF-κB pathway and various MAP kinase cascades. Fn14 expression is limited in adult organisms but becomes strongly induced in non-hematopoietic cells by a variety of growth factors, cytokines and physical stressors (e.g., hypoxia, irradiation). Since all these Fn14-inducing factors are frequently also present in the tumor microenvironment, Fn14 is regularly found to be expressed by non-hematopoietic cells of the tumor microenvironment and most solid tumor cells. In general, there are three possibilities how the tumor-Fn14 linkage could be taken into consideration for tumor therapy. First, by exploitation of the cancer associated expression of Fn14 to direct cytotoxic activities (antibody-dependent cell-mediated cytotoxicity (ADCC), cytotoxic payloads, CAR T-cells) to the tumor, second by blockade of potential protumoral activities of the TWEAK/Fn14 system, and third, by stimulation of Fn14 which not only triggers proinflammtory activities but also sensitizes cells for apoptotic and necroptotic cell death. Based on a brief description of the biology of the TWEAK/Fn14 system and Fn14 signaling, we discuss the features of the most relevant Fn14-targeting biologicals and review the preclinical data obtained with these reagents. In particular, we address problems and limitations which became evident in the preclinical studies with Fn14-targeting biologicals and debate possibilities how they could be overcome.
Antibody-based soluble and membrane-bound TWEAK mimicking agonists with FcγR-independent activity

Fibroblast growth factor (FGF)-inducible 14 (Fn14) activates the classical and alternative NFκB (nuclear factor ‘kappa-light-chain-enhancer’ of activated B-cells) signaling pathway but also enhances tumor necrosis factor (TNF)-induced cell death. Fn14 expression is upregulated in non-hematopoietic cells during tissue injury and is also often highly expressed in solid cancers. In view of the latter, there were and are considerable preclinical efforts to target Fn14 for tumor therapy, either by exploiting Fn14 as a target for antibodies with cytotoxic activity (e.g. antibody-dependent cellular cytotoxicity (ADCC)-inducing IgG variants, antibody drug conjugates) or by blocking antibodies with the aim to interfere with protumoral Fn14 activities. Noteworthy, there are yet no attempts to target Fn14 with agonistic Fc effector function silenced antibodies to unleash the proinflammatory and cell death-enhancing activities of this receptor for tumor therapy. This is certainly not at least due to the fact that anti-Fn14 antibodies only act as effective agonists when they are presented bound to Fcγ receptors (FcγR). Thus, there are so far no antibodies that robustly and selectively engage Fn14 signaling without triggering unwanted FcγR-mediated activities. In this study, we investigated a panel of variants of the anti-Fn14 antibody 18D1 of different valencies and domain architectures with respect to their inherent FcγR-independent ability to trigger Fn14-associated signaling pathways. In contrast to conventional 18D1, the majority of 18D1 antibody variants with four or more Fn14 binding sites displayed a strong ability to trigger the alternative NFκB pathway and to enhance TNF-induced cell death and therefore resemble in their activity soluble (TNF)-like weak inducer of apoptosis (TWEAK), one form of the natural occurring ligand of Fn14. Noteworthy, activation of the classical NFκB pathway, which naturally is predominately triggered by membrane-bound TWEAK but not soluble TWEAK, was preferentially observed with a subset of constructs containing Fn14 binding sites at opposing sites of the IgG scaffold, e.g. IgG1-scFv fusion proteins. A superior ability of IgG1-scFv fusion proteins to trigger classical NFκB signaling was also observed with the anti-Fn14 antibody PDL192 suggesting that we identified generic structures for Fn14 antibody variants mimicking soluble and membrane-bound TWEAK.
