The role of multicellularity as the predominant microbial lifestyle has been affirmed by studies on the genetic regulation of biofilms and the conditions driving their formation. Biofilms are of prime importance for the pathology of chronic infections of the opportunistic human pathogen Staphylococcus aureus.
The recent development of a macrocolony biofilm model in S. aureus opened new opportunities to study evolution and physiological specialization in biofilm communities in this organism. In the macrocolony biofilm model, bacteria form complex aggregates with a sophisticated spatial organization on the micro- and macroscale. The central positive and negative regulators of this organization in S. aureus are the alternative sigma factor σB and the quorum sensing system Agr, respectively. Nevertheless, nothing is known on additional factors controlling the macrocolony morphogenesis.
In this work, the genome of S. aureus was screened for novel factors that are required for the development of the macrocolony architecture. A central role for basic metabolic pathways was demonstrated in this context as the macrocolony architecture was strongly altered by the disruption of nucleotide and carbohydrate synthesis. Environmental signals further modulate macrocolony morphogenesis as illustrated by the role of an oxygen-sensitive gene regulator, which is required for the formation of complex surface structures. A further application of the macrocolony biofilm model was demonstrated in the study of interstrain interactions. The integrity of macrocolony communities was macroscopically visibly disturbed by competitive interactions between clinical isolates of S. aureus.
The results of this work contribute to the characterization of the macrocolony biofilm model and improve our understanding of developmental processes relevant in staphylococcal infections. The identification of anti-biofilm effects exercised through competitive interactions could lead to the design of novel antimicrobial strategies targeting multicellular bacterial communities.
Background
This study presents the results of a multidisciplinary, nosocomial MRSA outbreak investigation in an 8-bed medical intensive care unit (ICU). The identification of seven MRSA positive patients in the beginning of 2014 led to the closure of the ward for several weeks. A multidisciplinary, retrospective investigation was initiated in order to identify the reason and the source for the outbreak, describe MRSA transmission in the department and identify limitations in infection control.
Methods
The investigation comprised an epidemiological description of MRSA cases from 2012 to 2014 and a characterization of MRSA isolates, including phage-, spa- and PFGE-typing. Additionally, MRSA screening was performed from the hospital staff and the environment. To identify the reason for the outbreak, work-related, psychological and behavioral factors were investigated by impartial audits and staff interviews.
Results
Thirty-one MRSA cases were registered during the study period, and 36 isolates were investigated. Molecular typing determined the outbreak strain (phage type 54/812, PFGE type A4, spa type t003) and identified the probable index case. Nasal carriage in one employee and a high environmental contamination with the outbreak strain was documented. Important gaps in nursing procedures and general management were identified. Elevated stress levels and communication problems preceded the outbreak. Compliance with hand hygiene and isolation procedures was evaluated as appropriate.
Conclusion
This study demonstrates the complexity of controlling hospital-associated infections. The combined use of different typing methods is beneficial for outbreak investigations. Psychological, behavioral and other work-related factors have an important impact on the spread of nosocomial pathogens. These factors should be addressed and integrated in routine infection control practice.
In dieser Arbeit wurde das in vitro Wachstumsverhalten ausgesuchter MRSA in Konkurrenz zu Bakterien der Standortflora unter Optimalbedingungen und unter Mangelbedingungen getestet. Es lässt sich für alle getesteten MRSA-Stämme zusammenfassend sagen, dass ihre klinische Prävalenz nicht mit dem Wachstum in vitro korreliert, d.h. das häufige Spa-Typen nicht besser unter unseren Versuchbedingungen gewachsen sind als seltene. In vitro konnte kein verdrängendes Wachstum des Methicillin sensiblen S. aureus gegenüber den resistenten Stämme beobachtet werden. Vielmehr gelingt es den MRSA-Stämmen, ein Wachstumsgemisch zu ihrem Vorteil zu beeinflussen, indem sie die getesteten anderen Mikroorganismen (S. epidermidis, S. cerivisiae) im Wachstum hemmen, mit Ausnahme von E. faecium. Die Arbeit beleuchtet die Schwierigkeiten der Identifizierung von probiotischen Arten zur Verdrängung eines MRSA. In Zukunft sollte vielleicht an der Optimierung von in vitro Systemen gearbeitet werden (in vitro Organkulturen) oder Tiermodelle verwendet werden. Den Transmissionsunterschieden und der Tenazität sind weiterhin Aufmerksamkeit zu widmen. Wie in der Literatur beschrieben ist es zum Verständnis des Wachstumsverhal-tens der resistenten Stämme wichtig zu wissen, auf welchen molekularbiologischen Grundlagen die Resistenz beruht, da eine einzelne Site-Mutation zusätzliche Resistenzen bedeuten und einen eventuellen Wachstumsnachteil wieder ausgleichen kann. Im Klinikalltag scheinen sich die MRSA-Stämme auszubreiten, die den Wachstumsnachteil bereits ausgeglichen haben, beziehungsweise deren Methicillinresistenz keinen Wachstumsnachteil bedeutet.
Hintergrund: Zunehmend wird der Eigenschaft von Staphylococcus aureus als fakultativ intrazellulärem Erreger Bedeutung zugemessen. Ein direkter Nachweis der in vivo Relevanz von fakultativ intrazellulärem S. aureus bleibt allerdings bisher aus. Der Mechanismus zellulärer Invasivität ist bekannt und korreliert mit verschiedenen molekularen Markern (spa-Typ, SCCmec-Typ und pls/Pls). In dieser Studie wurde die Zuverlässigkeit und Ausweitbarkeit dieser Marker getestet. Des Weiteren wurde überprüft, ob sich die zelluläre Invasivität von kolonisierenden und Infektions-assoziierten MRSA-Isolaten unterscheidet und, ob die alleinige Bestimmung molekularer Marker in vitro die Virulenz eines Isolats in vivo abzuschätzen vermag. Methoden:Insgesamt wurden 109 MRSA-Isolate gesammelt, molekular charakterisiert (spa-Typ, BURP-Analyse, SCCmec-Typ, pls, agr-Typ, Hämolyseverhalten) und das Potential zellulärer Invasivität in vitro ermittelt. Die Assoziation eines Isolates mit einer Infektion in vivo wurde nachverfolgt (93 Kolonisierer versus 16 Infektions-assoziierte-Isolate). Zusätzlich wurde eine Referenzgruppe aus 13 S. aureus-Isolaten etabliert, die klinisch mit vergleichsweise invasiven Infektionen assoziiert waren (12 Osteomyelitis-Isolate und 1 Endokarditis-Isolat). Ergebnisse: Die bekannten molekularen Marker zellulärer Invasivität korrelieren zuverlässig in einer Population klinischer MRSA-Isolate und lassen sich auch auf bisher nicht bekannte (spa- und SCCmec-) Typen ausweiten. Das Hämolyseverhalten korrelierte nicht mit der zellulären Invasivität. Der agr-Typ wurde als weiterer molekularer Marker identifiziert. Die zelluläre Invasivität war unabhängig von der Etablierung einer Infektion in vivo (mediane Invasivität der Kolonisierer 100% versus 108% der Infektions-assoziierten Studienisolate und 110% der externen Referenzisolate). Des Weiteren waren die molekularen Marker spa- und agr-Typ nicht in der Lage, die Virulenz eines MRSA-Isolats in vivo abzuschätzen. Diskussion: Die zelluläre Invasivität klinischer MRSA-Isolate korreliert zuverlässig mit molekularen Markern. Allerdings vermögen weder die zelluläre Invasivität, noch mit ihr assoziierte molekulare Marker die Etablierung einer Infektion in vivo vorherzusagen. Beide scheinen also als Surrogat-Parameter zur Abschätzung der klinischen Virulenz eines Isolats ungeeignet. Zur Klärung der Frage, ob molekulare Marker zellulärer Invasivität in anderen Abschnitten der Pathogenese von S. aureus- Infektionen eine Rolle spielen, bedarf es weiterer Studien.
Introduction: Although there has been a worldwide emergence and spread of methicillin-resistant Staphylococcus aureus (MRSA), little is known about the molecular epidemiology of MRSA in Tanzania.
Methodology: In this study, we characterized MRSA strains isolated from clinical specimens at the Bugando Medical Centre, Tanzania, between January and December 2008. Of 160 S. aureus isolates from 600 clinical specimens, 24 (15%) were found to be MRSA. Besides molecular screening for the Panton Valentine leukocidin (PVL) genes by PCR, MRSA strains were further characterized by Multi-Locus Sequence Typing (MLST) and spa typing.
Results: Despite considerable genetic diversity, the spa types t690 (29.1%) and t7231 (41.6%), as well as the sequence types (ST) 88 (54.2%) and 1797 (29.1%), were dominant among clinical isolates. The PVL genes were detected in 4 isolates; of these, 3 were found in ST 88 and one in ST1820. Resistance to erythromycin, clindamicin, gentamicin, tetracycline and co-trimoxazole was found in 45.8%, 62.5%, 41.6%, 45.8% and 50% of the strains, respectively.
Conclusion: We present the first thorough typing of MRSA at a Tanzanian hospital. Despite considerable genetic diversity, ST88 was dominant among clinical isolates at the Bugando Medical Centre. Active and standardized surveillance of nosocomial MRSA infection should be conducted in the future to analyse the infection and transmission rates and implement effective control measures.
Background
Antimicrobial resistance has been declared by the World Health Organization as a threat to the public health. The aim of this study was to analyze antimicrobial resistance patterns of the common pathogens occurring at the Bugando Medical Centre (BMC), Mwanza, Tanzania to provide data for antimicrobial stewardship programmes.
Methods
A total of 3330 microbiological culture results scripts representing non-repetitive specimens reported between June 2013 and May 2015 were retrieved and analyzed for pathogens and their susceptibility patterns using STATA-11 software.
Results
Out of 3330 specimens, 439 (13.2%) had positive culture. Staphylococcus aureus (n = 100; 22.8%), Klebsiella pneumoniae (n = 65; 14.8%) and Escherichia coli (n = 41; 9.3%) were the most frequently isolated bacteria. Of 78 Staphylococcus aureus tested, 27 (34.6%) were found to be methicillin resistant Staphylococcus aureus (MRSA). Rates of resistance of Klebsiella pneumoniae and Escherichia coli isolates to third generation cephalosporins were 38.5% (25/65) and 29.3% (12/41) respectively. Staphylococcus aureus and Klesbiella pneumoniae were commonly isolated from bloodstream infections while Escherichia coli and Pseudomonas aeruginosa were the predominant isolates from urinary tract and wounds infections respectively. Of 23 Salmonella species isolated, 22 (95%) were recovered from the blood. Nine of the 23 Salmonella species isolates (39%) were found to be resistant to third generation cephalosporins. The resistance rate of gram-negative bacteria to third generation cephalosporins increased from 26.5% in 2014 to 57.9% in 2015 (p = 0.004) while the rate of MRSA decreased from 41.2% in 2013 to 9.5% in 2015 (p = 0.016). Multidrug-resistant gram-negative isolates were commonly isolated from Intensive Care Units and it was noted that, the majority of invasive infections were due to gram-negative bacteria.
Conclusion
There is an increase in proportion of gram-negative isolates resistant to third generation cephalosporins. The diversity of potential pathogens resistant to commonly prescribed antibiotics underscores the importance of sustained and standardized antimicrobial resistance surveillance and antibiotic stewardship programmes in developing countries.
Die Therapie von bakteriellen Infektionen beruht heutzutage zum Großteil auf dem Einsatz von Antibiotika. Die schnelle Entwicklung und rasche Verbreitung von resistenten Stämmen mancher Erreger gegen diese Antibiotika stellt ein enormes Problem für das Gesundheitswesen dar. Da momentan zur Antibiotikatherapie keine Alternativen bestehen, kommt der Erforschung neuer potenzieller Wirkstoffe eine sehr große Bedeutung zu. In einem Screening-Verfahren lagen die minimalen Hemmkonzentrationen einiger bisquartärer Bisnaphthalimide gegen Staphylococcus aureus und S. epidermidis im Bereich von 0,6 bis 2,5 µg/ml. Die Substanz mit den geringsten minimalen Hemmkonzentrationen war MT02. Daraufhin wurde das Wirkungsspektrum von MT02 gegen Bakterien detaillierter untersucht und gefunden, dass die Substanz vorwiegend gegen Gram-positive Erreger und nicht gegen Gram-negative Bakterien wirksam ist. Zytotoxizitätstests ergaben eine geringe bis nicht nachweisbare Toxizität gegen verschiedene Zelllinien im Bereich von 73 bis mehr als 150 µg/ml. Um die Wirkungsweise von MT02 genauer zu untersuchen wurden zunächst DNA-Microarray-Untersuchungen an S. aureus durchgeführt. Deren Ergebnisse ließen einen Einfluss der Substanz auf viele Gene des DNA-Metabolismus erkennen. Inkorporationsstudien mittels radioaktiver Ganzzellmarkierung bestätigten die Auswirkung von MT02 auf den DNA-Stoffwechsel. Durch kompetitive Inkubation wurde festgestellt, dass MT02 in der Lage ist Ethidiumbromid von DNA zu verdrängen bzw. dessen Bindung zu verhindern. Genauere Untersuchungen mittels Oberflächen-Plasmon-Resonanz ergaben, dass MT02 konzentrationsabhängig, reversibel und sequenzunspezifisch an DNA bindet. Die thermodynamischen Dissoziationskonstanten lagen im Mittel bei ca. 4 x 10-8 mol/l und beschrieben somit eine relativ starke Bindung von MT02 an DNA. Neben diesem primären Wirkungsmechanismus der DNA-Bindung gaben mehrere Befunde Hinweise auf einen sekundären Wirkmechanismus, der die Zellwand-Struktur bzw. Zellwand-Biosynthese beinhaltet. Eine MT02-resistente Mutante von S. aureus HG001 konnte durch vielfaches Passagieren in MT02-haltigem Medium generiert werden. Diese erzeugte bei Wachstum mit hohen Konzentrationen an MT02 einen roten Phänotyp. Die Natur dieses roten Farbstoffes konnte bislang nicht aufgeklärt werden, jedoch gibt es Hinweise, dass dieser auf Abbauprodukte von MT02 zurückzuführen ist. In einem weiteren Projekt wurde mittels Transkriptionsstudien die Auswirkung von verschiedenen bekannten Antibiotika sowie von neuen Wirkstoffen auf das Transkriptom von S. epidermidis untersucht. Die Ergebnisse dieser Studien können durch vergleichende Analysen als Grundlage für die Einordnung des Wirkmechanismus neuer Substanzen dienen.
Infektionen durch MRSA können aufgrund der zunehmenden Therapieresistenz der Erreger ernsthafte Verläufe zeigen. Daher ist die Entwicklung alternativer Behandlungsstrategien ein Ziel der aktuellen Infektionsforschung. Ein vielversprechender Ansatz liegt in der Verwendung pathogenspezifischer, monoklonaler Antikörper gegen immunodominante Antigene der Erreger. Durch die Fortschritte in der Genomforschung der vergangenen Jahre ist nun die Analyse der Gesamtheit der Pathogenitätsfaktoren eines bakteriellen Erregers möglich. Durch eine funktionelle Genomanalyse mit Hilfe immunologischer Detektionssysteme können antigene Bakterienzellstrukturen identifiziert werden. Daraus abgeleitete Targetstrukturen sollen die Grundlage bilden für die Entwicklung spezifischer humaner Antikörper, die in alternativen Therapieansätzen zusätzlich zur antimikrobiellen Chemotherapie zukünftig an Bedeutung gewinnen werden. In der vorliegenden Arbeit wurde eine Expressionsgenbank eines MRSA-Stammes hergestellt und mittels Patientenseren nach immunodominanten Antigenen gesucht. Die aus einem Partialverdau der genomischen DNA gewonnenen Fragmente wurden in einen Expressionsvektor kloniert. Die so hergestellte Genbank des ausgewählten MRSA-Stammes A 134 umfasst ca. 10000 Klone. Ein vergleichendes Screening der Genbank erfolgte mit dem Serum eines Patienten mit einer MRSA-Sepsis und mit dem Serum einer negativen Kontrollperson. Die DNA-Inserts der immunoreaktiven Klone wurden sequenziert und durch Datenbankvergleiche identifiziert. Die Ergebnisse dieser Arbeit zeigen deutlich, dass der gewählte Ansatz geeignet ist, um immunodominante Antigene, die während einer Sepsis exprimiert werden, zu identifizieren. Die in der vorliegenden Arbeit gefundenen putativen immunodominanten Antigenstrukturen umfassen ein Holin, die Amidase LytA, Faktoren des isd-Genclusters, der für die Cadmiumresistenz wichtige Transporter CadA, unbekannte Proteine der Staphylokokken-Pathogenitätsinsel SaPI3 und Protein A. Interessanterweise wurden durch den methodischen Ansatz vorwiegend Proteine auf mobilen genetischen Elementen identifiziert. Diese Ergebnisse legen nahe, dass während einer S. aureus-Infektion nicht nur Gene für das Wachstum der Zellen und Virulenzfaktoren wie Toxine und Adhäsine, sondern auch mobile Elemente exprimiert werden. Die Bedeutung dieser Prozesse für das Infektionsgeschehen ist allerdings bislang unbekannt und zukünftige Untersuchungen müssen zeigen, inwieweit diese Elemente für die Pathogenese von Bedeutung sind. Die in dieser Arbeit identifizierten Antigene müssen auch in zukünftigen Arbeiten durch Subklonierung weiter charakterisiert werden. Eine weitere interessante Beobachtung ist die Identifizierung eines Bakteriophagens, der in das isd-Gencluster inserierte, das für die Aufnahme von Eisen eine Bedeutung besitzt. Inwieweit durch die Insertion des Phagens die Eisenaufnahme in dem klinischen Isolat gestört wird, müssen zukünftige Studien zeigen. Die positive Reaktion des Patientenserums mit dem Protein A ist am wahrscheinlichsten auf die Antiköperbindung am FC-Fragment zurückzuführen. Es ist jedoch auch möglich, dass während der Infektion verstärkt Protein A gebildet wird, das durch die Bindung von Antikörpern am Fc-Teil eine immunsuppressive Wirkung durch die Neutralisierung opsonisierender und Toxin-inaktivierender Antikörper besitzt. Durch die Sequenzierung und Datenbankanalyse der positiven Klone konnte ein erster Überblick über immunodominante Antigene von S. aureus erhalten werden. Da auf den Insertelementen meist mehrere putative Antigene kodiert sind, müssen in zukünftigen Arbeiten die identifizierten Insertelemente subkloniert und damit weiter charakterisiert werden. Dadurch sollte es möglich sein, die immunogenen Strukturen zu erkennen und diese für die Entwicklung monoklonaler Antikörper zu nutzen.
Novel Approaches to Antimicrobial Therapy of Pneumonia using Antibiotics and Therapeutic Antibodies

Nosocomial pneumonia is mostly caused by methicillin-resistant Staphylococcus aureus (MRSA). However, the standard antibiotic therapy is affected by increasing emergence of bacterial resistance. Therefore, novel therapeutic options are in high demand. New antimicrobial agents alone cannot handle the problem of increasing bacterial resistance but innovative drug delivery strategies and fast identification of infection causing pathogens are required to diminish bacterial resistance development. A very promising approach to improve the therapy of pneumonia is presented by local drug delivery to the lung. This application method enables high local drug concentrations in the lung leading to shorter application of antibiotics and hence reduces the risk of resistance development. Furthermore, the systemic concentration is lowered reducing the emergence of adverse effects.
Therefore, in this thesis several approaches to improve the therapy of MRSA pneumonia are studied.
One approach to achieve an efficient local delivery of antibiotics are nano-sized drug delivery systems which enable the nebulization of poorly-soluble antibiotics and can lead to even higher local drug concentrations due to their small size since nanoparticles improve mucus penetration and decrease phagocytosis by alveolar macrophages. Here, an analytical setup was developed that facilitates the identification of optimal preparation conditions for drug polyelectrolyte nanoplexes.
Another promising approach to support antimicrobial therapy of pneumonia is presented by antibody-based immunotherapy. Since the stability of the antibody and hence its therapeutic activity are endangered during production, transport, storage, and application, a stabilizing formulation was developed for hUK-66, an antibody targeting surface antigens of S. aureus. Furthermore, nebulization of this formulated monoclonal antibody was studied to enable local application. Finally, the immunotherapeutic efficacy of the nebulized hUK-66 formulation was investigated in an animal in vivo study.
Furthermore, rapid identification of the infection triggering pathogen is very important. The selective detection of S. aureus was achieved using optical planar Bragg grating sensors functionalized with hUK-66. In addition, the reusability of this system was studied applying a surface functionalization based on the cross-linker SPDP which enables a reversible fixation of the antibody.
