Background
Adequate anticoagulation is prerequisite for effective hemodialysis to prevent clotting in the extracorporeal circuit. We aimed providing first data on the efficacy and safety of the low-molecular-weight heparin certoparin in this setting.
Methods
Multicenter, open-label, 8-week trial. Patients received a single dose of 3,000 IU certoparin i.v. with additional titration steps of 600 IU and/or continuous infusion if necessary.
Results
120 patients were screened, 109 enrolled (median age 71; range 26–90 years) and 106 available for efficacy analyses. The percentage of unsatisfactory dialysis results at 8 weeks due to clotting or bleeding, was 1.9% (n = 2/106; 95% confidence interval [CI] 0.23–6.65%); no major bleeding. 1.9% had moderate/severe clotting in the lines/bubble catcher and 2.8% in the dialyser at week 8. 15.7 ± 14.3% of the dialysis filters’ visual surface area was showing redness. In subgroups of patients receiving median doses of 3000 ± 0, 3000 (2400–6000) and 4200 (3000–6600) IU, plasma aXa levels at baseline, 4 and 8 weeks were 0.24 [95%CI 0.21–0.27], 0.33 [0.27–0.40] and 0.38 [0.33–0.45] aXa IU/ml at 2 h. \(C_{48h}\) was 0.01 [0.01–0.02] aXa IU at all visits. At baseline and 4 weeks \(AUC_{0-48h}\) was 2.66 [2.19–3.24] and 3.66 [3.00–4.45] aXa IU*h/ml. In 3.0% of dialyses (n = 83/2724) prolonged fistula compression times were documented. Eight patients (7.34%) had at least one episode of minor bleeding. 4) 85.3% of patients had any adverse event, 9.2% were serious without suspected drug relation; and in 32 patients a drug-relation was suspected.
Conclusions
Certoparin appears effective and safe for anticoagulation in patients undergoing maintenance hemodialysis.
Protein binding prevents uremic toxins from removal by conventional extracorporeal therapies leading to accumulation in maintenance dialysis patients. Weakening of the protein binding may enhance the dialytic elimination of these toxins. In ultrafiltration and equilibrium dialysis experiments, different measures to modify the plasma binding affinity and capacity were tested: (i), increasing the sodium chloride (NaCl) concentration to achieve a higher ionic strength; (ii), increasing the temperature; and (iii), dilution. The effects on the dissociation constant K-D and the protein bound fraction of the prototypical uremic toxin indoxyl sulfate (IS) in plasma of healthy and uremic individuals were studied. Binding of IS corresponded to one site binding in normal plasma. K-D increased linearly with the NaCl concentration between 0.15 (K-D = 13.2 +/- 3.7 mu M) and 0.75 M (K-D = 56.2 +/- 2.0 mu M). Plasma dilution further reduced the protein bound toxin fraction by lowering the protein binding capacity of the plasma. Higher temperatures also decreased the protein bound fraction of IS in human plasma. Increasing the NaCl concentration was effective to weaken the binding of IS also in uremic plasma: the protein bound fraction decreased from 89% +/- 3% to 81% +/- 3% at 0.15 and 0.75 M NaCl, respectively. Dilution and increasing the ionic strength and temperature enhance the free fraction of IS allowing better removal of the substance during dialysis. Applied during clinical dialysis, this may have beneficial effects on the long-term outcome of maintenance dialysis patients.
Introduction
Medium-cut-off (MCO) dialyzers may beneficially impact outcomes in patients on hemodialysis.
Methods
In a randomized, controlled trial in maintenance hemodialysis patients, the new Nipro ELISIO-17HX MCO dialyzer was compared to the Baxter Theranova 400 filter regarding middle molecule removal. Furthermore, the suitability of two assays for free lambda-light chain (λFLC) detection (Freelite vs. N-Latex) was verified.
Results
ELISIO-HX achieved slightly lower reduction ratios for β2-microglobulin (71.8 ± 6.0 vs. 75.3 ± 5.8%; p = 0.001), myoglobin (54.7 ± 8.6 vs. 64.9 ± 8.7%; p < 0.001), and kappa-FLC (62.1 ± 8.8 vs. 56.3 ± 7.7%; p = 0.021). λFLC reduction ratios were more conclusive with the Freelite assay and not different between ELISIO-HX and Theranova (28.4 ± 3.9 vs. 38.7 ± 13.4%; p = 0.069). The albumin loss of Theranova was considerably higher (2.14 ± 0.45 vs. 0.77 ± 0.25 g; p = 0.001) and the Global Removal ScoreLoss alb largely inferior (30.6 ± 7.4 vs. 82.4 ± 29.2%/g; p = 0.006) to ELISIO-HX.
Conclusions
The new ELISIO-HX expands the choice of dialyzers for MCO hemodialysis.
Introduction Patients with end-stage kidney disease requiring maintenance haemodialysis treatment experience a dramatic cardiovascular morbidity and mortality. Due to the high atherosclerotic and arteriosclerotic burden and profound alterations in haemostasis, they frequently suffer and die from both thromboembolic and bleeding events. This is a particular concern in patients on haemodialysis with atrial fibrillation (AF). Controlled trials on the optimal anticoagulation in patients with AF on haemodialysis are not available. The randomised controlled phase IIIb AXADIA-AFNET 8 trial will evaluate the safety and efficacy of the factor Xa inhibitor apixaban in patients with AF requiring haemodialysis. Methods and analysis A total of 222 patients will be randomised in an open-labelled, 1:1 design to receive either apixaban 2.5mg twice daily or dose-adjusted vitamin K antagonist therapy (target international normalised ratio 2.0-3.0). All patients will be treated and followed up for a minimum of 6 months up to a maximum of 24 months. The primary outcome is major or clinically relevant, non-major bleedings or death of any cause. Secondary outcomes include stroke, cardiovascular death and other thromboembolic events, thus exploring the efficacy of apixaban. The first patient was randomised in June 2017. Ethics and dissemination The study protocol was approved by the Ethical Committee of the Landesaertzekammer, Westfalen-Lippe and the Medical Faculty of the University of Muenster, Muenster, Germany (reference number: 2016-598f-A). Written informed consent will be obtained from all patients prior to study participation, including their consent for long-term follow-up. AXADIA-AFNET 8 is an investigator-initiated trial. Sponsor is AFNET, Muenster, Germany. Study findings will be disseminated to Bristol-Myers Squibb, Munich, Germany, and Pfizer, Berlin, Germany, to the participating centres, at research conferences and in peer-reviewed journals. The protein-bound uremic toxins para-cresyl sulfate (pCS) and indoxyl sulfate (IS) are associated with cardiovascular disease in chronic renal failure, but the effect of different dialysis procedures on their plasma levels over time is poorly studied. The present prospective, randomized, cross-over trial tested dialysis efficacy and monitored pre-treatment pCS and IS concentrations in 15 patients on low-flux and high-flux hemodialysis and high-convective volume postdilution hemodiafiltration over six weeks each. Although hemodiafiltration achieved by far the highest toxin removal, only the mean total IS level was decreased at week three (16.6 ± 12.1 mg/L) compared to baseline (18.9 ± 13.0 mg/L, p = 0.027) and to low-flux dialysis (20.0 ± 12.7 mg/L, p = 0.021). At week six, the total IS concentration in hemodiafiltration reached the initial values again. Concentrations of free IS and free and total pCS remained unaltered. Highest beta2-microglobulin elimination in hemodiafiltration (p < 0.001) led to a persistent decrease of the plasma levels at week three and six (each p < 0.001). In contrast, absent removal in low-flux dialysis resulted in rising beta2-microglobulin concentrations (p < 0.001). In conclusion, this trial demonstrated that even large differences in instantaneous protein-bound toxin removal by current extracorporeal dialysis techniques may have only limited impact on IS and pCS plasma levels in the longer term.
Background
In patients undergoing maintenance hemodialysis (HD), increased levels of circulating fibroblast growth factor-23 (FGF-23) are independently associated with cardiovascular events and mortality. Interventional strategies aiming to reduce levels of FGF-23 in HD patients are of particular interest. The purpose of the current study was to compare the impact of high-flux versus low-flux HD on circulating FGF-23 levels.
Methods
We conducted a post-hoc analysis of the MINOXIS study, including 127 dialysis patients randomized to low-flux (n = 62) and high-flux (n = 65) HD for 52 weeks. Patients with valid measures for FGF-23 investigated baseline and after 52 weeks were included.
Results
Compared to baseline, a significant increase in FGF-23 levels after one year of low-flux HD was observed (Delta plasma FGF-23: +4026 RU/ml; p < 0.001). In contrast, FGF-23 levels remained stable in the high flux group (Delta plasma FGF-23: +373 RU/ml, p = 0.70). The adjusted difference of the absolute change in FGF-23 levels between the two treatment groups was statistically significant (p < 0.01).
Conclusions
Over a period of 12 months, high-flux HD was associated with stable FGF-23 levels, whereas the low-flux HD group showed an increase of FGF-23. However, the implications of the different FGF 23 time-trends in patients on high flux dialysis, as compared to the control group, remain to be explored in specifically designed clinical trials.
In dieser post-hoc Analyse der Deutschen Diabetes und Dialyse Studie wurde der Einfluss von NT-proBNP und Troponin T auf plötzlichen Herztod, Schlaganfall, Myokardinfarkt und die Gesamtmortalität während vierjähriger Studiendauer bei 1255 Patienten mit Diabetes mellitus Typ 2 an der Hämodialyse analysiert. Des Weiteren wurde die Bedeutung einer longitudinalen Messung der Biomarker nach 6 Monaten auf die Endpunkte untersucht. Patienten mit dem höchsten NT-proBNP respektive Troponin T wiesen die größte Ereignisrate für plötzlichen Herztod, Schlaganfall und die Gesamtmortalität auf. In der multivariaten Regressionsanalyse waren sowohl NT-proBNP als auch Troponin T jeweils starke unabhängige Prädiktoren für plötzlichen Herztod, Schlaganfall und die Gesamtmortalität. Eine Assoziation von NT-proBNP mit dem Auftreten von Myokardinfarkten wurde nicht gesehen. Nicht nur ein hoher Ausgangswert der Biomarker, sondern auch eine Zunahme von NT-proBNP und Troponin T nach 6 Monaten waren assoziiert mit einer schlechteren Langzeitprognose
Hintergrund: Der Einfluss von Dialysemembranen auf Morbidität und Mortalität von Hämodialysepatienten ist hoch kontrovers. Methoden: Es erfolgte eine post-hoc Analyse der 4D Studie, einer randomisierten, placebo-kontrollierten, multizentrischen Studie, die die Wirkung von Atorvastatin bei hämodialysepflichtigen Typ 2 Diabetikern untersucht hat. Vor Analyse der Daten wurden die Patienten je nach verordneten Dialysefiltern in Gruppen eingeteilt; entsprechend Permeabilität und Flux wurden high- und low-flux und entsprechend der Biokompatibilität wurden Cellulose-, semisynthetische und synthetische Dialysemembranen unterschieden. Von 1255 Patienten wurden 648 (52%) identifiziert, die kontinuierlich mit einer synthetischen (syn) high-flux (HFS) (n=214), einer low-flux (LFS) syn (n=247), einer semisynthetischen LF (LFSS) (n=119) oder einer LF Cellulose (LFC)-Membran (n=41) behandelt wurden. Die Endpunkte waren Gesamtmortalität und kombinierte kardiale Mortalität, bestehend aus nicht tödlichem Herzinfarkt, Apoplex und kardiovaskulärem Tod. Ergebnisse: Nach einer Beobachtungszeit von 4 Jahren wurden die multivariaten relativen Risiken (RR) berechnet und adjustiert. Das RR den kardiovaskulären Endpunkt zu erreichen war signifikant höher für Patienten, die mit LFC. (RR 2.33; 95% Konfidenzintervall [CI] 1.38-3.94; p=0.002), LFSS (RR 1.92; 95% CI 1.35-2.73; p=0.0003) oder LFS (RR 1.35; 95% CI, 0.99-1.85; p=0.06) Membranen dialysiert wurden, als für Patienten, die mit HFS behandelt wurden. Entsprechend war das RR zu sterben höher bei Dialyse mit LFC (RR 4.14; 95 %CI 2.79-6.15; p<0.0001 ) LFSS (RR 2.24; 95% CI 1.66-3.02; p<0.0001), LFS (RR 0.45; 95% CI 1.22-2.07; p=0.0006) als bei Dialyse mit HFS . Auch im Vergleich zu Patienten, die mit LFS behandelt wurden, zeigte sich für LFC ein um 161% höheres RR (95% CI 1.80-3.79; p<0.0001) und für LFSS ein um 41% höheres RR (95% CI 1.07-1.86; p=0.016) zu sterben. Schließlich war bei Vergleich von LFC und LFSS das RR zu sterben bei Patienten, die mit LFC dialysiert wurden um 85% höher (95% CI 1.24-2.76; p=0.0025). Die geringste Rate an Todesfällen zeigte sich in der HFS-Gruppe; die kumulative Inzidenz betrug 12% nach 1 und 33% nach 3 Jahren. Am höchsten war die Gesamtmortalität in der LFC-Gruppe mit 44% und 84% nach 1 bzw. 3 Jahren. Gleichfalls signifikante Unterschiede zeigten sich für die Vergleiche mit LFS und LFSS. Ein ähnliches Bild ergibt sich bei der Betrachtung der Kaplan Meier Überlebenskurven für den kardiovaskulären Endpunkt: Die kumulative Inzidenz betrug für HFS 12,3% nach 1 und 30,5% nach 3 Jahren und für LFC 24,3% nach 1 und 60,8% nach 3 Jahren. Schlussfolgerung: Biokompatibilität und Flux von Dialysemembranen haben Einfluss auf die Prognose hämodialysepflichtiger Typ 2 diabetischer Patienten.
Einfluß der Dialysetherapie auf den Genomschaden von Nierenpatienten in einer prospektiven Studie

Patienten mit terminaler Niereninsuffizienz haben im Vergleich zur Normalbevölkerung eine deutlich erhöhte Inzidenz maligner Erkrankungen. Frühere Untersuchungen zeigten, dass periphere Blutlymphozyten dieser Patienten einen höheren genetischen Schaden aufweisen, wodurch das Risiko einer malignen Entartung steigt. In dieser Arbeit wurde der genetische Schaden mithilfe zweier Testverfahren, Comet Assay und Mikrokerntest, untersucht. Es handelte sich um eine prospektive Studie mit zwei Patientenkollektiven. Die erste Gruppe bestand aus Patienten, die aufgrund einer terminalen Niereninsuffizienz innerhalb der nächsten Monate eine Dialysetherapie mittels konventioneller Hämodialyse beginnen mußten. Die zweite Gruppe bildeten Dialysepatienten, die im Verlauf von konventioneller Dialyse auf Hämodiafiltration umgestellt wurden. Bei allen Patienten wurde der genetische Schaden der peripheren Blutlymphozyten in den Monaten vor und nach Therapiebeginn bzw. Therapieumstellung regelmäßig untersucht. Unsere Ergebnisse zeigen, dass 4 der 10 Prädialysepatienten nach Beginn der Dialyse einen niedrigeren genetischen Schaden hatten, 2 Patienten hatten unterschiedliche Werte in Comet Assay und Mikrokerntest und bei 2 Patienten ergab sich im Verlauf eine höhere DNA-Schädigung. Die verbliebenen 2 Patienten mußten aufgrund einer konstant bleibenden Niereninsuffizienz nicht mit der Dialysetherapie beginnen. Bei Zusammenfassung aller Einzelwerte zeigte sich, dass das Kollektiv der Prädialysepatienten insgesamt vom Beginn der Behandlung profitiert hat. In der Gruppe der Dialysepatienten hatte 2 von 7 Patienten nach Umstellung auf Hämodiafiltration eine geringere DNA-Schädigung, 2 Patienten zeigten unterschiedliche Ergebnisse im Comet Assay und Mikrokerntest und 2 weitere Patienten wiesen eine höheren genetischen Schaden in den Lymphozyten auf. Ein Patient konnte bei fehlenden Vorwerten nicht berücksichtigt werden. Im Gruppenvergleich zeigte sich für alle Dialysepatienten ein gleichbleibender DNA-Schaden, gemessen mithilfe des Comet Assays bei leicht erhöhten Mikrokernraten. Jedoch hatte sich die Zellproliferation ebenfalls etwas verbessert. Zusammenfassend ergibt sich somit in beiden Gruppen kein eindeutiges Ergebnis, woraus neue Therapieempfehlungen für Patienten mit terminaler Niereninsuffizienz abzuleiten wären. Um weiter Einflußvariablen auf die Höhe des genetischen Schadens festzustellen, sind weiter Untersuchungen mit größeren Patientenkollektiven erforderlich.
Zirkulierende endotheliale Vorläuferzellen, kurz EPCs, werden als Biomarker für kardiovaskuläre Erkrankungen genutzt. In einer prospektiven, randomisierten Cross-over-Studie wurden 18 chronische Dialysepatienten während jeweils vier Wochen mit Low-Flux-HD, High-Flux-HD sowie Hämodiafilatration behandelt. EPCs wurden zu Beginn sowie am Ende jedes Intervalls bestimmt. Trotz deutlicher Unterschiede bezüglich der Toxinentfernung im mittelmolekularen Bereich zeigte sich kein Einfluss auf Qualität und Quantität der EPCs
