Aims
Vitamin D deficiency is prevalent in heart failure (HF), but its relevance in early stages of heart failure with preserved ejection fraction (HFpEF) is unknown. We tested the association of 25-hydroxyvitamin D [25(OH)D] serum levels with mortality, hospitalizations, cardiovascular risk factors, and echocardiographic parameters in patients with asymptomatic diastolic dysfunction (DD) or newly diagnosed HFpEF.
Methods and results
We measured 25(OH)D serum levels in outpatients with risk factors for DD or history of HF derived from the DIAST-CHF study. Participants were comprehensively phenotyped including physical examination, echocardiography, and 6 min walk test and were followed up to 5 years. Quality of life was evaluated by the Short Form 36 (SF-36) questionnaire. We included 787 patients with available 25(OH)D levels. Median 25(OH)D levels were 13.1 ng/mL, mean E/e′ medial was 13.2, and mean left ventricular ejection fraction was 59.1%. Only 9% (n = 73) showed a left ventricular ejection fraction <50%. Fifteen per cent (n = 119) of the recruited participants had symptomatic HFpEF. At baseline, participants with 25(OH)D levels in the lowest tertile (≤10.9 ng/L; n = 263) were older, more often symptomatic (oedema and fatigue, all P ≤ 0.002) and had worse cardiac [higher N-terminal pro-brain natriuretic peptide (NT-proBNP) and left atrial volume index, both P ≤ 0.023], renal (lower glomerular filtration rate, P = 0.012), metabolic (higher uric acid levels, P < 0.001), and functional (reduced exercise capacity, 6 min walk distance, and SF-36 physical functioning score, all P < 0.001) parameters. Increased NT-proBNP, uric acid, and left atrial volume index and decreased SF-36 physical functioning scores were independently associated with lower 25(OH)D levels. There was a higher risk for lower 25(OH)D levels in association with HF, DD, and atrial fibrillation (all P ≤ 0.004), which remained significant after adjusting for age. Lower 25(OH)D levels (per 10 ng/mL decrease) tended to be associated with higher 5 year mortality, P = 0.05, hazard ratio (HR) 1.55 [1.00; 2.42]. Furthermore, lower 25(OH)D levels (per 10 ng/mL decrease) were related to an increased rate of cardiovascular hospitalizations, P = 0.023, HR = 1.74 [1.08; 2.80], and remained significant after adjusting for age, P = 0.046, HR = 1.63 [1.01; 2.64], baseline NT-proBNP, P = 0.048, HR = 1.62 [1.01; 2.61], and other selected baseline characteristics and co-morbidities, P = 0.043, HR = 3.60 [1.04; 12.43].
Conclusions
Lower 25(OH)D levels were associated with reduced functional capacity in patients with DD or HFpEF and were significantly predictive for an increased rate of cardiovascular hospitalizations, also after adjusting for age, NT-proBNP, and selected baseline characteristics and co-morbidities.
Herzinsuffizienz ist eine sehr häufige Erkrankung vor allem des höheren Lebensalters. Biomarker wie NT-proBNP, BNP, hsCRP haben neben ihrer Bedeutung für die Diagnose einer akuten Herzinsuffizienz einen großen Stellenwert in der Abschätzung der Prognose eines Patienten. Die prognostische Relevanz dieser Marker konnte auch bei nicht herzinsuffizienten, anderweitig kranken Patienten gezeigt werden. Unklar und wenig erforscht ist die Aussagekraft von Biomarkern in einem Kollektiv nicht akut dekompensierter Patienten, welche sich ambulant bei ihrem Hausarzt vorstellen. Die Handheld-BNP-Studie untersuchte im primärärztlichen Bereich das diagnostische Potential von BNP und der miniaturisierten Echokardiographie. Die vorliegende Follow-up-Studie untersucht die prognostische Relevanz von BNP sowie vergleichend den prognostischen Wert von NT-proBNP und der Kardiologendiagnose. Auch die prognostische Aussagekraft der inflammatorischen Marker hsCRP und TNF-ɑ, ebenso wie die Frage, ob durch eine Kombination der Marker die prognostische Abschätzung weiter gesteigerter werden kann, ist Gegenstand dieser Arbeit. Zuletzt wurde eine multivariate Regressionsanalyse durchgeführt, um den unabhängigen prognostischen Wert der Biomarker zu untersuchen. Es konnte gezeigt werden, dass bei diagnostisch naiven Patienten mit dem klinisch-anamnestischen Verdacht auf das Vorliegen einer Herzinsuffizienz das kardiale wie auch das nicht-kardiale Mortalitätsrisiko sowie die Rate an Hospitalisierungen gegenüber der Allgemeinbevölkerung gleichen Alters erhöht sind, unabhängig vom Vorliegen einer Herzinsuffizienz. Eine Bestimmung der Biomarker BNP, NT-proBNP, hsCRP und TNF-ɑ erwies sich in diesem Kollektiv als hilfreich, diejenigen mit erhöhtem Risiko zu erkennen.
Mutations in mitochondrial aminoacyl-tRNA synthetases (mtARSs) have been reported in patients with mitochondriopathies: most commonly encephalopathy, but also cardiomyopathy. Through a GWAS, we showed possible associations between mitochondrial valyl-tRNA synthetase (VARS2) dysregulations and non-ischemic cardiomyopathy. We aimed to investigate the possible consequences of VARS2 depletion in zebrafish and cultured HEK293A cells. Transient VARS2 loss-of-function was induced in zebrafish embryos using Morpholinos. The enzymatic activity of VARS2 was measured in VARS2-depleted cells via northern blot. Heterozygous VARS2 knockout was established in HEK293A cells using CRISPR/Cas9 technology. BN-PAGE and SDS-PAGE were used to investigate electron transport chain (ETC) complexes, and the oxygen consumption rate and extracellular acidification rate were measured using a Seahorse XFe96 Analyzer. The activation of the integrated stress response (ISR) and possible disruptions in mitochondrial fatty acid oxidation (FAO) were explored using RT-qPCR and western blot. Zebrafish embryos with transient VARS2 loss-of-function showed features of heart failure as well as indications of CNS and skeletal muscle involvements. The enzymatic activity of VARS2 was significantly reduced in VARS2-depleted cells. Heterozygous VARS2-knockout cells showed a rearrangement of ETC complexes in favor of complexes III\(_2\), III\(_2\) + IV, and supercomplexes without significant respiratory chain deficiencies. These cells also showed the enhanced activation of the ISR, as indicated by increased eIF-2α phosphorylation and a significant increase in the transcript levels of ATF4, ATF5, and DDIT3 (CHOP), as well as disruptions in FAO. The activation of the ISR and disruptions in mitochondrial FAO may underlie the adaptive changes in VARS2-depleted cells.
Die Herzinsuffizienz ist eine der führenden Todesursachen weltweit. Eine auf die neurohumorale Aktivierung zugeschnittene Therapie mit ACE-Hemmern bzw. Angiotensin-Rezeptorantagonisten, Betablockern, Aldosteronantagonisten und Diuretika verbessert zwar die Symptomatik und Prognose. Letztere ist bei Diagnosestellung jedoch immer noch schlechter als die vieler maligner Erkrankungen einzuschätzen. Ziel ist daher die Entwicklung von Medikamenten, die den Krankheitsverlauf des Syndroms Herzinsuffizienz aufhalten bzw. umkehren. Ein Ansatz ist dabei die Analyse sogenannter Kandidatengene, die im kranken Herzen differentiell exprimiert werden und potentiell medikamentös beeinflussbar sind. Im Rahmen der vorliegenden Arbeit wurden zwei solcher Kandidatengene charakterisiert. Mäuse mit kardialer Überexpression b1-adrenerger Rezeptoren entwickeln eine Herzinsuffizienz mit kardialer Hypertrophie und Fibrose. In Gen Arrays mit 21000 Maus-ESTs zeigten sich unter anderem die Gene des Uncoupling Protein 2 (UCP2) und der Diphosphoinositol-Polyphosphat-Phosphohydrolase 2 alpha (DIPP2a) aktiviert. Diese Befunde wurden zunächst mittels RNase Protection Assay und RT-PCR bestätigt. Auch andere murine Herzinsuffizienzmodelle wurden untersucht. So ließ sich ebenfalls im druckinduzierten Herzinsuffizienzmodell nach artifizieller Aortenstenose sowie im b2-AR überexprimierenden Herzen eine erhöhte Konzentration von UCP2- und DIPP2a-mRNA messen. Um zu prüfen, ob diese differentielle mRNA-Expression deletäre oder protektive Effekte vermittelt, wurden jeweils transgene Mauslinien mit herzspezifischer Überexpression von UCP2 und DIPP2a generiert. Die Linie UCP2-TG1 mit hoher Überexpression sowie ein Gründer-Tier der UCP2-transgenen Mäuse entwickelten eine Herzinsuffizienz mit vergrößertem Herzen, linksventrikulärem Pumpversagen, interstitieller Fibrose und typischen Veränderungen der molekularen Marker ANF und SERCA. Zudem fanden sich dilatierte Vorhöfe sowie eine bradykarde Herzrhythmusstörung. UCP2 ist ein Entkoppler der oxidativen Phosphorylierung im Mitochondrium. Im energieintensiven Stoffwechsel des Myokards könnte eine durch UCP2 reduzierte ATP-Synthese zu den genannten Veränderungen führen. Für UCP2 wurden auch protektive Eigenschaften durch das Abfangen freier Radikale beschrieben. Die Linie UCP2-TG3 mit niedrigerem Überexpressionsniveau und ohne kardialen Phänotyp wurde deshalb einem Aortic banding unterzogen, wo sich in der Überlebenskurve kein protektiver oder deletärer Effekt einer moderat vermehrten Entkopplung im Vergleich zum Wildtyp zeigte. In drei unabhängigen Linien transgener Mäuse mit herzspezifischer Überexpression von DIPP2a ließ sich morphometrisch eine kardiale Hypertrophie nachweisen. Die Linie DIPP2a-TG9 mit dem höchsten Überexpressionsniveau zeigte zudem eine kardiale Fibrose sowie unter Dobutamin eine verminderte kardiale Kontraktilitätsreserve im Vergleich zum Wildtypen. DIPP-Proteine hydrolysieren Inositolphosphate und Nukleosiddiphosphate und greifen so in zentrale Stoffwechselvorgänge ein, die im einzelnen noch nicht geklärt sind. Es konnte hier im Mausmodell gezeigt werden, dass UCP2 und DIPP2a zwei für die Entwicklung einer Herzinsuffizienz relevante Zielproteine darstellen. Geplant ist die weitere Aufklärung der beteiligten Mechanismen, um diese letztlich auch therapeutisch beeinflussen zu können.
Transgene Mausmodelle zur Charakterisierung der Funktion kardialer beta-adrenerger Rezeptoren

In der vorliegenden Arbeit wurde die Funktion kardialer beta-adrenerger Rezeptoren mit Hilfe einer Kombination aus transgenen Mausmodellen und physiologischen und molekularbiologischen Methoden untersucht. Durch gezielte Überexpression des humanen beta1-adrenergen Rezeptors im Herzen transgener Mäuse konnte gezeigt werden, daß die chronische Aktivierung dieses Rezeptors eine trophische Wirkung auf die Herzmuskelzellen hat. Über einen Zeitraum von mehreren Monaten führte dies zur Entwicklung einer Herzinsuffizienz. In der menschlichen Herzinsuffizienz kommt es zu einem ähnlichen Phänomen: Durch deutlich erhöhte Freisetzung von endogenen Katecholaminen kommt es zu einer chronischen Dauerstimulation kardialer beta1-adrenerger Rezeptoren. Daß diese schädlich ist belegen das hier beschriebene Mausmodell und zudem einige neuere klinische Studien, die zeigen daß eine pharmakologische Blockade beta-adrenerger Rezeptoren zu einer Verminderung der Herzinsuffizienzmortalität führt. Dieses Mausmodell erlaubte es erstmals den beta1-adrenergen Rezeptor hinsichtlich seiner spontanen Rezeptoraktivität in einem physiologischen Modell zu untersuchen. Dabei zeigte sich, daß der humane beta1-adrenerge Rezeptor spontane Aktivität aufweist, jedoch in einem deutlich geringeren Ausmaß als der beta2-adrenerge Rezeptor. Dies könnte klinisch relevant sein, da klinisch verwendete beta-Rezeptor-Antagonisten die spontane Aktivität des beta1-adrenergen Rezeptors in unserem Modell unterschiedlich stark unterdrückten. In der vorliegenden Arbeit wurde zudem untersucht, ob sich die beiden kardial exprimierten Beta-Rezeptor-Subtypen Beta1 und Beta2 hinsichtlich ihrer Signaltransduktion unterscheiden. Ausgehend von dem Befund, daß die chronische Aktivierung der beiden Subtypen in transgenen Mausmodellen zu deutlich unterschiedlichen Phänotypen führt, wurden verschiedene intrazelluläre Signalwege auf ihre Aktivierung hin überprüft. Abweichend von publizierten, in vitro nach kurzzeitiger Rezeptorstimulation erhobenen Daten zeigte sich, daß die chronische Aktivierung der Rezeptorsubtypen zu einer unterschiedlichen Aktivierung der kardialen MAP-kinasen (ERK) führt. Die beta1-spezifische Aktivierung dieser Kinasen könnte die beobachtete unterschiedliche Hypertrophieentwicklung in diesen beiden Mausmodellen erklären. Einen weiteren Schwerpunkt bei der Aufklärung des Mechanismus beta-adrenerg induzierter Hypertrophie bildete die Untersuchung der zellulären Calcium-homöostase. Als früheste funktionelle Veränderung in der Entwicklung einer beta-adrenerg induzierten Herzhypertrophie und -insuffizienz trat dabei eine Störung des intrazellulären Calciumtransienten auf. Als möglicher Mechanismus für die Störung des Calciumhaushalts konnte eine zeitgleich auftretende veränderte Expression des Calcium-regulierenden Proteins Junctin beschrieben werden. Einen neuen therapeutischen Ansatz für die Therapie der Herzinsuffizienz könnten schließlich vielleicht die Untersuchungen zum kardialen Na/H-austauscher ergeben: Es konnte erstmals gezeigt werden, daß der kardiale Na/H-Austauscher maßgeblich an der beta-adrenerg induzierten Herzhypertrophie- und Fibrose-entstehung beteiligt ist und daß die pharmakologische Inhibition dieses Proteins sowohl Hypertrophie als auch die Fibrose wirksam unterdrücken kann.
Fibrosis is a pivotal player in heart failure development and progression. Measurements of (markers of) fibrosis in tissue and blood may help to diagnose and risk stratify patients with heart failure, and its treatment may be effective in preventing heart failure and its progression. A lack of pathophysiological insights and uniform definitions has hampered the research in fibrosis and heart failure. The Translational Research Committee of the Heart Failure Association discussed several aspects of fibrosis in their workshop. Early insidious perturbations such as subclinical hypertension or inflammation may trigger first fibrotic events, while more dramatic triggers such as myocardial infarction
and myocarditis give rise to full blown scar formation and ongoing fibrosis in diseased hearts. Aging itself is also associated with a cardiac phenotype that includes fibrosis. Fibrosis is an extremely heterogeneous phenomenon, as several stages of the fibrotic process exist, each with different fibrosis subtypes and a different composition of various cells and proteins — resulting in a very complex pathophysiology. As a result, detection of fibrosis, e.g. using current cardiac imaging modalities or plasma biomarkers, will detect only specific subforms of fibrosis, but cannot capture all aspects of the complex fibrotic process. Furthermore, several anti-fibrotic therapies are under investigation, but such therapies generally target aspecific aspects of the fibrotic process and suffer from a lack of precision. This review discusses the mechanisms and the caveats and proposes a roadmap for future research.
OBJECTIVES: This study evaluated the tolerability and feasibility of titration of 2 distinctly acting beta-blockers (BB) in elderly heart failure patients with preserved (HFpEF) and reduced (HFrEF) left ventricular ejection fraction.
BACKGROUND: Broad evidence supports the use of BB in HFrEF, whereas the evidence for beta blockade in HFpEF is uncertain.
METHODS: In the CIBIS-ELD (Cardiac Insufficiency Bisoprolol Study in Elderly) trial, patients >65 years of age with HFrEF (n = 626) or HFpEF (n = 250) were randomized to bisoprolol or carvedilol. Both BB were up-titrated to the target or maximum tolerated dose. Follow-up was performed after 12 weeks. HFrEF and HFpEF patients were compared regarding tolerability and clinical effects (heart rate, blood pressure, systolic and diastolic functions, New York Heart Association functional class, 6-minute-walk distance, quality of life, and N-terminal pro-B-type natriuretic peptide).
RESULTS: For both of the BBs, tolerability and daily dose at 12 weeks were similar. HFpEF patients demonstrated higher rates of dose escalation delays and treatment-related side effects. Similar HR reductions were observed in both groups (HFpEF: 6.6 beats/min; HFrEF: 6.9 beats/min, p = NS), whereas greater improvement in NYHA functional class was observed in HFrEF (HFpEF: 23% vs. HFrEF: 34%, p < 0.001). Mean E/e' and left atrial volume index did not change in either group, although E/A increased in HFpEF. CONCLUSIONS: BB tolerability was comparable between HFrEF and HFpEF. Relevant reductions of HR and blood pressure occurred in both groups. However, only HFrEF patients experienced considerable improvements in clinical parameters and Left ventricular function. Interestingly, beta-blockade had no effect on established and prognostic markers of diastolic function in either group. Long-term studies using modern diagnostic criteria for HFpEF are urgently needed to establish whether BB therapy exerts significant clinical benefit in HFpEF. Diabetes mellitus is a common disease affecting more than 537 million adults worldwide. The microvascular complications that occur during the course of the disease are widespread and affect a variety of organ systems in the body. Diabetic retinopathy is one of the most common long-term complications, which include, amongst others, endothelial dysfunction, and thus, alterations in the blood-retinal barrier (BRB). This particularly restrictive physiological barrier is important for maintaining the neuroretina as a privileged site in the body by controlling the inflow and outflow of fluid, nutrients, metabolic end products, ions, and proteins. In addition, people with diabetic retinopathy (DR) have been shown to be at increased risk for systemic vascular complications, including subclinical and clinical stroke, coronary heart disease, heart failure, and nephropathy. DR is, therefore, considered an independent predictor of heart failure. In the present review, the effects of diabetes on the retina, heart, and kidneys are described. In addition, a putative common microRNA signature in diabetic retinopathy, nephropathy, and heart failure is discussed, which may be used in the future as a biomarker to better monitor disease progression. Finally, the use of miRNA, targeted neurotrophin delivery, and nanoparticles as novel therapeutic strategies is highlighted.
Alzheimer's disease (AD), the most common cause of dementia in the elderly, is a neurodegenerative disorder associated with neurovascular dysfunction and cognitive decline. While the deposition of amyloid β peptide (Aβ) and the formation of neurofibrillary tangles (NFTs) are the pathological hallmarks of AD-affected brains, the majority of cases exhibits a combination of comorbidities that ultimately lead to multi-organ failure. Of particular interest, it can be demonstrated that Aβ pathology is present in the hearts of patients with AD, while the formation of NFT in the auditory system can be detected much earlier than the onset of symptoms. Progressive hearing impairment may beget social isolation and accelerate cognitive decline and increase the risk of developing dementia. The current review discusses the concept of a brain–ear–heart axis by which Aβ and NFT inhibition could be achieved through targeted supplementation of neurotrophic factors to the cochlea and the brain. Such amyloid inhibition might also indirectly affect amyloid accumulation in the heart, thus reducing the risk of developing AD-associated amyloid cardiomyopathy and cardiovascular disease.
Background: \(^{18}\)F-N-[3-bromo-4-(3-fluoro-propoxy)-benzyl]-guanidine (\(^{18}\)F-LMI1195) is a new class of PET tracer designed for sympathetic nervous imaging of the heart. The favorable image quality with high and specific neural uptake has been previously demonstrated in animals and humans, but intracellular behavior is not yet fully understood. The aim of the present study is to verify whether it is taken up in storage vesicles and released in company with vesicle turnover.
Results: Both vesicle-rich (PC12) and vesicle-poor (SK-N-SH) norepinephrine-expressing cell lines were used for in vitro tracer uptake studies. After 2 h of \(^{18}\)F-LMI1195 preloading into both cell lines, effects of stimulants for storage vesicle turnover (high concentration KCl (100 mM) or reserpine treatment) were measured at 10, 20, and 30 min. \(^{131}\)I-meta-iodobenzylguanidine (\(^{131}\)I-MIBG) served as a reference. Both high concentration KCl and reserpine enhanced \(^{18}\)F-LMI1195 washout from PC12 cells, while tracer retention remained stable in the SK-N-SH cells. After 30 min of treatment, 18F-LMI1195 releasing index (percentage of tracer released from cells) from vesicle-rich PC12 cells achieved significant differences compared to cells without treatment condition. In contrast, such effect could not be observed using vesicle-poor SK-N-SH cell lines. Similar tracer kinetics after KCl or reserpine treatment were also observed using 131I-MIBG. In case of KCl exposure, Ca\(^{2+}\)-free buffer with the calcium chelator, ethylenediaminetetracetic acid (EDTA), could suppress the tracer washout from PC12 cells. This finding is consistent with the tracer release being mediated by Ca\(^{2+}\) influx resulting from membrane depolarization.
Conclusions: Analogous to \(^{131}\)I-MIBG, the current in vitro tracer uptake study confirmed that \(^{131}\)F-LMI1195 is also stored in vesicles in PC12 cells and released along with vesicle turnover. Understanding the basic kinetics of \(^{18}\)FLMI1195 at a subcellular level is important for the design of clinical imaging protocols and imaging interpretation.
