Neurofibromatosis type 1 (NF1) is a genetic multisystemic autosomal dominant disorder determining reduced life expectancy due to higher risk of developing benign and malignant tumors. Low levels of vitamin D and reduced bone mineral density (BMD) have been reported in young patients with NF1. However, correlation between vitamin D and NF1 phenotype needs to be elucidated. Aim of this study was to assess vitamin D levels and bone metabolism in NF1 patients, analyzing potential correlations with clinical phenotype. A cross-sectional study was carried out in a monocentric series of NF1 patients, evaluating genotype, clinical phenotype, BMD, biochemical evaluation with focus on serum 25OH-vitamin D, parathyroid hormone (PTH), calcium and phosphate levels. Correlations between clinical manifestations, neurofibromas, and vitamin D status have been studied in comparison with healthy controls. 31 NF1 adult patients were matched for sex, age and body mass index with 31 healthy controls. A significantly difference in vitamin D level emerged in NF1 patients compared to controls. Interestingly low vitamin D levels correlated with a more aggressive phenotype and with a bigger size of neurofibromas. These data underline that vitamin D deficiency/insufficiency may play a role in clinical severity of neurofibromas in patients with NF1, suggesting the need to check bone status and replace vitamin D in these patients.
The glycoprotein sclerostin has been identified as a negative regulator of bone growth. It exerts its function by interacting with the Wnt co-receptor LRP5/6, blocks the binding of Wnt factors and thereby inhibits Wnt signalling. Neutralizing anti-sclerostin antibodies are able to restore Wnt activity and enhance bone growth thereby presenting a new osteoanabolic therapy approach for diseases such as osteoporosis. We have generated various Fab antibodies against human and murine sclerostin using a phage display set-up. Biochemical analyses have identified one Fab developed against murine sclerostin, AbD09097 that efficiently neutralizes sclerostin's Wnt inhibitory activity. In vitro interaction analysis using sclerostin variants revealed that this neutralizing Fab binds to sclerostin's flexible second loop, which has been shown to harbour the LRP5/6 binding motif. Affinity maturation was then applied to AbD09097, providing a set of improved neutralizing Fab antibodies which particularly bind human sclerostin with enhanced affinity. Determining the crystal structure of AbD09097 provides first insights into how this antibody might recognize and neutralize sclerostin. Together with the structure–function relationship derived from affinity maturation these new data will foster the rational design of new and highly efficient anti-sclerostin antibodies for the therapy of bone loss diseases such as osteoporosis.
Context
Primary aldosteronism (PA) is the most frequent form of endocrine hypertension. Besides its deleterious impact on cardiovascular target organ damage, PA is considered to cause osteoporosis.
Patients and methods
We assessed bone turnover in a subset of 36 postmenopausal women with PA. 18 patients had unilateral PA and were treated by adrenalectomy, whereas 18 patients had bilateral PA and received mineralocorticoid receptor antagonist (MRA) therapy respectively. 18 age- and BMI-matched females served as controls. To estimate bone remodeling, we measured the bone turnover markers intact procollagen 1 N-terminal propeptide, bone alkaline phosphatase, osteocalcin and tartrate resistant acid phosphatase 5b in plasma by chemiluminescent immunoassays at time of diagnosis and one year after initiation of treatment.

Setting
Tertiary care hospital.
Results
Compared with controls, patients with PA had mildly elevated osteocalcin at baseline (p = 0.013), while the other bone markers were comparable between both groups. There were no differences between the unilateral and the bilateral PA subgroup. One year after initiation of MRA treatment with spironolactone bone resorption and bone formation markers had significantly decreased in patients with bilateral PA. In contrast, patients adrenalectomized because of unilateral PA showed no significant change of bone turnover markers.
Conclusion
This study shows that aldosterone excess in postmenopausal women with PA is not associated with a relevant increase of bone turnover markers at baseline. However, we observed a significant decrease of bone markers in patients treated with spironolactone, but not in patients treated by adrenalectomy.
Mit einer Prävalenz von 15% der Gesamtbevölkerung und sogar über 30% bei den Frauen über dem 50. Lebensjahr zählt die Osteoporose zu den Erkrankungen welcher bei steigender Lebenserwartung zunehmend sozioökonomisches Gewicht zukommt. Die Folgekosten bei Komplikationen wie Frakturen nach Stürzen übersteigen die einer Therapie um einiges. Ein Motor der Entstehung von Osteoporose ist oxidativer Stress. Dieser regt über Enzymkaskaden die Bildung und Aktivierung von Osteoklasten an, was einen erhöhten Knochenabbau zur Folge hat. Zu den körpereigenen Verteidigungstrategien gegen Oxidation gehören Selenoproteine wie Glutathionperoxidasen. In einer vorangegangenen Arbeit von Prof.Jakob und C. Becker wurde bereits ein Zusammenhang der GPx-Aktivität mit der Ausbildung von Osteoporose nachgewiesen. Osteoporotiker hatten eine signifikant verminderte Aktivität dieses Enzyms. Ein essentielles Spurenelement für die Herstellung der Gluthationperoxidasen ist Selen. Deutschland gilt diesbezüglich als Mangelversorgungsgebiet. Es ist bekannt, dass ein Mangel an Selen Krankheiten begünstigen kann. Als Beispiele seien hier die Keshan-Krankheit oder Kashin-Beck- Krankheit genannt. Auch in der Intensivmedizin wurde ein Nutzen der Selengabe bei Sepsispatienten nachgewiesen. Die Selenspiegel aller eingeschlossenen 163 Probanden wurden untersucht. Es zeigte sich durchgehend eine leichte Erniedrigung. Signifikante Unterschiede ergaben sich nicht. Ein mutmaßlicher Mangel an Selen war als Ursache für eine verminderte Gluthationperoxidaseaktivität nicht nachzuweisen. Da der Hauptproduktionsort der GPx die Niere ist, wurde als Arbeitsthese die Vermutung aufgestellt, dass eine Niereninsuffizienz über eine verminderte Produktion dieser antioxidativ wirkenden Enzyme als Ursache einer Osteoporose in Frage kommt. Berechnet wurde die Niereninsuffizienz in Form der GFR nach der Cockroft-Gault- Formel. Es ergab sich eine signifikante Reduzierung der GFR zwischen Kontrolle und Osteoporosegruppe (p<0,05) nicht aber zwischen Kontrolle und Osteopeniegruppe und Osteopenie- und Osteoporosegruppe. Bezüglich des Alters und GFR konnte eine signifikante Korrelation nachgewiesen werde. Zwischen der GPx-Aktivität und der glomerulären Filtrationsrate bestand eine signifikante Korrelation ( Korrelation nach Pearson 0.149 mit einer Signifikanz von 0.018; Spearman-Rho 0.162 mit Signifikanz 0.010 beides p< 0,05). Es bestätigt sich also die in der Literatur bekannte positive Assoziation der gestörten Nierenfunktion mit der Manifestation der Osteoporose. Mit der GPx wird in dieser ersten Arbeit ein Sekretionsprodukt der Niere identifiziert, welches über eine Verminderung der Sekretion in der gestörten Nierenfunktion die antioxidative Kapazität des Organismus einschränkt und damit die Entstehung einer Osteoporose begünstigen könnte.
Sarcopenia and Malnutrition Screening in Female Osteoporosis Patients — A Cross-Sectional Study

Sarcopenia and malnutrition are important determinants of increased fracture risk in osteoporosis. SARC-F and MNA-SF are well-established questionnaires for identifying patients at risk for these conditions. We sought to evaluate the feasibility and potential added benefit of such assessments as well as the actual prevalence of these conditions in osteoporosis patients. We conducted a cross-sectional, single-center study in female osteoporosis patients ≥ 65 years (SaNSiBaR-study). Results of the sarcopenia (SARC-F) and malnutrition (MNA-SF) screening questionnaires were matched with a functional assessment for sarcopenia and data from patients’ medical records. Out of 107 patients included in the analysis, a risk for sarcopenia (SARC-F ≥ 4 points) and a risk for malnutrition (MNA-SF ≤ 11 points) was found in 33 (30.8%) and 38 (35.5%) patients, respectively. Diagnostic overlap with coincident indicative findings in both questionnaires was observed in 17 patients (16%). As compared to the respective not-at-risk groups, the mean short physical performance battery (SPPB) score was significantly reduced in both patients at risk for sarcopenia (7.0 vs. 10.9 points, p < 0.001) and patients at risk for malnutrition (8.7 vs. 10.5 points, p = 0.005). Still, confirmed sarcopenia according to EWGSOP2 criteria was present in only 6 (6%) of all 107 patients, with only 3 of them having an indicative SARC-F score. Bone mineral density was not significantly different in any of the at-risk groups at any site. In summary, applying SARC-F and MNA-SF in osteoporosis patients appears to be a complementary approach to identify individuals with functional deficits.

In dieser Arbeit wurden die Diagnostik- und Behandlungsabläufe von 250 Patienten nach erlittener proximaler Femurfraktur in der Region Würzburg (Deutschland) untersucht. Auswertungsschwerpunkte waren die durchgeführte Diagnostik zur Abklärung einer Osteoporose, die Einleitung einer Pharmakotherapie und die Informationsübermittlung an den weiterbehandelnden Arzt. Aus den erhobenen Daten konnte eine Inzidenz für die Jahre 1993 und 1994 von 180 und eine Inzidenzdichte auf 100.000 Einwohner von 138,5 pro Jahr gemeinsam für Frauen und Männer hochgerechnet werden. Das mittlere Alter der untersuchten Patienten lag bei 76,3 Jahren, die 10%-Perzentile bei 59, die 90%-Perzentile bei 89 Jahren und der Median war 80 Jahre, und damit vergleichbar mit den anderen internationalen Studien. Die geschlechtsspezifischen Verteilung der Frakturen zeigte ein deutliches Übergewicht der Frauen (194 vs. 56 bei Männern). Bei allen Patienten unterblieb eine weitere Abklärung der Frakturursache während des stationären Aufenthaltes, obwohl die Diagnose Osteoporose zumindest hoch wahrscheinlich (241 Fälle) oder stationär festgestellt worden war (147 Fälle, radiologisch oder histologisch). - In keinem Fall wurde die zur Differenzialdiagnose erforderliche Laborroutine vollständig durchgeführt. - In 147 Fällen wurde die Diagnose einer Osteoporose durch den Radiologen (konventionelle Röntgenaufnahme) oder durch den Pathologen (Untersuchung des Femurkopfes) gestellt (in 127 Fällen radiologisch, in 58 Fällen histopathologisch). - Bei nur 20 der so festgestellten 147 Fälle (13,6 %) wurde eine Osteoporose-Therapie stationär eingeleitet und in nur 13 Fällen als Therapieempfehlung für den Entlassungsbericht übernommen. - Wurde die Diagnose durch den Radiologen oder Pathologen gestellt, so unterblieb in 2 von 3 Fällen jegliche Erwähnung im Entlassungsbericht. Wurde sie erwähnt, dann häufig nur in der Form des Röntgen- oder Histologiebefunds. - Die Diagnose Osteoporose wurde in 19,6 % der Entlassungsbriefe übermittelt und lag damit um ca. 5 % höher als der internationale Vergleich. - Wäre die stationär in 147 Fällen bereits festgestellte Diagnose jedes Mal übermittelt worden, hätte sich statt 19,6 % eine Quote von 58,8 % erreichen lassen. Eine Schenkelhalsfraktur steigert die Morbidität und Mortalität der betroffenen Patienten erheblich. Lediglich 23 von zuvor 195 Patienten konnten bei Entlassung aus der Akutklinik ohne Hilfe gehen, während die Zahl der vollständig immobilen Patienten von 2 auf 23 Patienten zum Zeitpunkt der Entlassung zunahm. 14 Patienten (5,6 %) starben im Krankenhaus oder im dokumentierten Beobachtungszeitraum. 26 Patienten (10,4 %) erlitten bereits ihre zweite proximale Femurfraktur, 12 (4,8 %) davon innerhalb nur eines Jahres und zwei sogar ihre dritte proximale Femurfraktur (0,8%). Die für den Patienten wirkungsvollen und das Gesundheitssystem kosteneffektiven Behandlungsmöglichkeiten machen eine weiterführende diagnostische Abklärung und Behandlung der proximalen Femurfraktur aus ethischen und sozioökonomischen Gründen erforderlich. Dies betrifft den Arzt der Akutversorgung und den weiterbehandelnden Arzt gleichermaßen. Die Behandlung sollte multimodal unter Einschluss einer adäquaten Pharmakotherapie erfolgen. Die aktuellen Therapieempfehlungen lassen sich auch für den nicht Osteologen verständlich und praktikabel aus den aktuellen Leitlinien z.B. der Deutschen Gesellschaft für Osteologie entnehmen und anwenden. Zu möglichen nicht medikamentösen Maßnahmen gehören Behandlungskonzepte mit Mobilisationstraining (Fallverhütung), Hüftprotektoren und Reduktion/Vermeidung von Sedativa (v. a. Benzodiazepine). Das Bewusstsein von Ärzten und Patienten muss für den Zusammenhang „Fraktur mit inadäquatem Trauma“ und „Osteoporose“ geschärft werden. Fortbildungen und Öffentlichkeitsarbeit können hier wertvolle Dienste leisten. Jede erlittene Fraktur mit inadäquatem Trauma sollte bei Arzt und Patient die Frage nach einer Osteoporose aufwerfen. Eine weiterführende Abklärung sollte gegebenenfalls eingeleitet und die Notwendigkeit einer Behandlung überprüft werden. - Diese Studie belegt, dass die Versorgung für den untersuchten Zeitraum völlig ungenügend ist. - Sie kann als Basis dienen, um Verbesserungen in diesem Bereich zu dokumentieren. - Sie zeigt, dass umfassende Anstrengungen erforderlich sind, das Bewusstsein für den Zusammenhang proximale Femurfraktur und Osteoporose zu schärfen und effektive Präventionsmaßnahmen (z.B. Verhinderung einer zweiten Schenkelhalsfraktur) einzuleiten.
Knowledge of local bone quality is essential for surgeons to determine operation techniques. A device for intraoperative measurement of local bone quality has been developed by the AO-Research Foundation (DensiProbe®). We used this device to experimentally measure peak breakaway torque of trabecular bone in the proximal femur and correlated this with local bone mineral density (BMD) and failure load. Bone mineral density of 160 cadaver femurs was measured by ex situ dual-energy X-ray absorptiometry. The failure load of all femurs was analyzed by side-impact analysis. Femur fractures were fixed and mechanical peak torque was measured with the DensiProbe® device. Correlation was calculated whereas correlation coefficient and significance was calculated by Fisher’s Z-transformation. Moreover, linear regression analysis was carried out. The unpaired Student’s t-test was used to assess the significance of differences. The Ward triangle region had the lowest BMD with 0.511 g/cm2 (±0.17 g/cm2), followed by the upper neck region with 0.546 g/cm2 (±0.16 g/cm2), trochanteric region with 0.685 g/cm2 (±0.19 g/cm2) and the femoral neck with 0.813 g/cm2 (±0.2 g/cm2). Peak torque of DensiProbe® in the femoral head was 3.48 Nm (±2.34 Nm). Load to failure was 4050.2 N (±1586.7 N). The highest correlation of peak torque measured by Densi Probe® and load to failure was found in the femoral neck (r=0.64, P<0.001). The overall correlation of mechanical peak torque with T-score was r=0.60 (P<0.001). A correlation was found between mechanical peak torque, load to failure of bone and BMD in vitro. Trabecular strength of bone and bone mineral density are different aspects of bone strength, but a correlation was found between them. Mechanical peak torque as measured may contribute additional information about bone strength, especially in the perioperative testing.
Background: Studies with extracellular vesicles (EVs), including exosomes, isolated from mesenchymal stem cells (MSC) indicate benefits for the treatment of musculoskeletal pathologies as osteoarthritis (OA) and osteoporosis (OP). However, little is known about intercellular effects of EVs derived from pathologically altered cells that might influence the outcome by counteracting effects from “healthy” MSC derived EVs. We hypothesize, that EVs isolated from osteoblasts of patients with hip OA (coxarthrosis/CA), osteoporosis (OP), or a combination of both (CA/OP) might negatively affect metabolism and osteogenic differentiation of bone-marrow derived (B)MSCs.
Methods: Osteoblasts, isolated from bone explants of CA, OP, and CA/OP patients, were compared regarding growth, viability, and osteogenic differentiation capacity. Structural features of bone explants were analyzed via μCT. EVs were isolated from supernatant of naïve BMSCs and CA, OP, and CA/OP osteoblasts (osteogenic culture for 35 days). BMSC cultures were stimulated with EVs and subsequently, cell metabolism, osteogenic marker gene expression, and osteogenic differentiation were analyzed.
Results: Trabecular bone structure was different between the three groups with lowest number and highest separation in the CA/OP group. Viability and Alizarin red staining increased over culture time in CA/OP osteoblasts whereas growth of osteoblasts was comparable. Alizarin red staining was by trend higher in CA compared to OP osteoblasts after 35 days and ALP activity was higher after 28 and 35 days. Stimulation of BMSC cultures with CA, OP, and CA/OP EVs did not affect proliferation but increased caspase 3/7-activity compared to unstimulated BMSCs. BMSC viability was reduced after stimulation with CA and CA/OP EVs compared to unstimulated BMSCs or stimulation with OP EVs. ALP gene expression and activity were reduced in BMSCs after stimulation with CA, OP, and CA/OP EVs. Stimulation of BMSCs with CA EVs reduced Alizarin Red staining by trend.
Conclusion: Stimulation of BMSCs with EVs isolated from CA, OP, and CA/OP osteoblasts had mostly catabolic effects on cell metabolism and osteogenic differentiation irrespective of donor pathology and reflect the impact of tissue microenvironment on cell metabolism. These catabolic effects are important for understanding differences in effects of EVs on target tissues/cells when harnessing them as therapeutic drugs.
Die Osteoporose des Mannes ist eine unterdiagnostizierte Erkrankung. Die Zahl der betroffenen Männer in Deutschland wird auf 0,8 bis 1 Million geschätzt, aber die reale Inzidenz ist wahrscheinlich höher. Die Osteoporose des Mannes ist Schätzungen zufolge in bis zu 60% mit Begleiterkrankungen assoziiert. Hypophysäre Störungen wie Hypogonadismus und Wachstumshormonmangel sind starke Risikofaktoren für die Entwicklung einer Osteoporose. Es gibt in der Literatur nur wenige Daten über Hypophysenerkrankungen und Osteoporose, abgesehen davon, dass bei bestehendem Wachstumshormonmangel die Assoziation der Osteopenie / Osteoporose gesichert ist. Für die nicht diagnostizierten Fälle von GH-Defizienz kann jedoch die Manifestation einer Osteoporose ein erstes und führendes Symptom sein. Es gibt Evidenzen dafür, dass IGF-1 im Serum in gesunden Populationen positiv mit der Knochendichte korreliert. Das Ziel dieser Studie ist, eine große Anzahl an Patienten mit Osteoporose auf die Inzidenz niedriger IGF-1-Spiegel hin zu untersuchen.
