In cats anesthetized with alpha-chloralose, extracellular recordings were made from fine afferent units belonging to the medial articular nerve (MAN) of the knee joint. The excitatory and sensitizing effects on articular afferents of serotonin (5-HT) applied intra-arterially close to the joint were examined. The joints were either normal or an experimental arthritis had been induced some hours before the recording session. Bolus injections of 1.35-135 p,g 5-HT excited about 43% of group 111 (CV: 2.5-20 m/sec) and 73% of group IV units (CV: < 2.5 mjsec) from normal joints. The latency was usually between 10 and 30 sec, and the duration and size of the responses were dose-dependent. Fast group 111 units (CV: > 16 mjsec) and group li units (CV: > 20 m/sec) were never excited by 5-HT. Repetitiveadministration led to pronounced tachyphylaxis of the 5-HT response. Inflammation induced an enhanced sensitivity of group III articular afferent units to close intra-arterial application of 5-HT. In particular the total duration of each response was considerably prolonged (4-10 min against 1-2 min under normal conditions). At the same time the tachyphylaxis seen under normal conditions was gteatly reduced. In contrast, group IV articular afferent units did not become sensitized to 5-HT in the course of inflammation. In normal joints 5-HT did not sensitize fineafferent units for movement-induced responses. However, after inflammation, a distinct sensitization to such movements by 5-HT application could be observed bothin group 111 and group IV fiber ranges. The sensitization had a short time course not exceeding 7 min. The tonic component of the movement-induced response was more enhanced than the phasic one. The bolus application of 5-HT led to temporary vasoconstriction of the knee joint vessels. This vasoconstriction was especially pronounced in inflamed joints and impeded the access of subsequently applied substances to the terminal regions of the afferent units under observation. lt is concluded that the present results support the notion that 5-HT may participate in the mediation of pain from inflamed tissue such as an arthritic joint by exciting and sensitizing fine afferent units. During inflammation group 111 units are particularly sensitive to 5-HT and, thus, may carry the bulk of the 5-HT-induced nociceptive messages.
In CHO-FucTVI- Zellen wurde hLysII/IV stabil transfiziert, mit Puromycin selektioniert und hLysII/IV-FucTVI von den stabil transfizierten CHO-FucTVI- Zelle überexprimiert. Durch Immunaffinitätschromatographie und Ultrafiltration wurde das überexprimierte hLysII/IV-FucTVI aufgereinigt und aufkonzentriert. Durch den Lysozymtest nach Osserman und Lawlor und einen ELISA konnte die Lysozymmenge in den unterschiedlichen Schritten bestimmt werden. Im anschließenden Zelladhäsionsassay konnten bei Konzentrationen von 1 ng/ml, 10 ng/ml und 100 ng/ml hLysII/IV-FucTVI signifikante Reduktionen der Zelladhäsion von U937- Zellen an HUVEC- Zellen festgestellt werden. Die ermittelte mittlere Hemmkonzentration (IC50) von hLysII/IV-FucTVI liegt bei 7*10-12 M. Dies entspricht bei einem Molekulargewicht von 30 kDa der Menge von 0,21 ng/ml und hLysII/IV-FucTVI wäre damit der stärkste bisher bekannte E-Selektin-Antagonist. In dieser Funktion könnte hLysII/IV-FucTVI im Rahmen einer antiinflammatischen oder antineoplastischen Therapie eingesetzt werden.
Background:
Inhibition of early platelet adhesion by blockade of glycoprotein-IB (GPIb) protects mice from ischemic stroke. To elucidate underlying mechanisms in-vivo, infarct development was followed by ultra-high field MRI at 17.6 Tesla.
Methods:
Cerebral infarction was induced by transient-middle-cerebral-artery-occlusion (tMCAO) for 1 hour in C57/BL6 control mice (N = 10) and mice treated with 100 mg Fab-fragments of the GPIb blocking antibody p0p/B 1 h after tMCAO (N = 10). To control for the effect of reperfusion, additional mice underwent permanent occlusion and received anti-GPIb treatment (N = 6; pMCAO) or remained without treatment (N = 3; pMCAO). MRI 2 h and 24 h after MCAO measured cerebral-blood-flow (CBF) by continuous arterial-spin labelling, the apparent-diffusion-coefficient (ADC), quantitative-T2 and T2-weighted imaging. All images were registered to a standard mouse brain MRI atlas and statistically analysed voxel-wise, and by cortico-subcortical ROI analysis.
Results:
Anti-GPIb treatment led to a relative increase of postischemic CBF vs. controls in the cortical territory of the MCA (2 h: 44.2 +/- 6.9 ml/100g/min versus 24 h: 60.5 +/- 8.4; p = 0.0012, F((1,18)) = 14.63) after tMCAO. Subcortical CBF 2 h after tMCAO was higher in anti-GPIb treated animals (45.3 +/- 5.9 vs. controls: 33.6 +/- 4.3; p = 0.04). In both regions, CBF findings were clearly related to a lower probability of infarction (Cortex/Subcortex of treated group: 35%/65% vs. controls: 95%/100%) and improved quantitative-T2 and ADC. After pMCAO, anti-GPIb treated mice developed similar infarcts preceded by severe irreversible hypoperfusion as controls after tMCAO indicating dependency of stroke protection on reperfusion.
Conclusion:
Blockade of platelet adhesion by anti-GPIb-Fab-fragments results in substantially improved CBF early during reperfusion. This finding was in exact spatial correspondence with the prevention of cerebral infarction and indicates in-vivo an increased patency of the microcirculation. Thus, progression of infarction during early ischemia and reperfusion can be mitigated by anti-platelet treatment.
Background:
During the last years, (19)F-MRI and perfluorocarbon nanoemulsion (PFC) emerged as a powerful contrast agent methodology to track cells and to visualize inflammation. We applied this new modality to visualize deep tissue abscesses during acute and chronic phase of inflammation caused by Staphylococcus aureus infection.
Methodology and Principal Findings:
In this study, a murine thigh infection model was used to induce abscess formation and PFC or CLIO (cross linked ironoxides) was administered during acute or chronic phase of inflammation. 24 h after inoculation, the contrast agent accumulation was imaged at the site of infection by MRI. Measurements revealed a strong accumulation of PFC at the abscess rim at acute and chronic phase of infection. The pattern was similar to CLIO accumulation at chronic phase and formed a hollow sphere around the edema area. Histology revealed strong influx of neutrophils at the site of infection and to a smaller extend macrophages during acute phase and strong influx of macrophages at chronic phase of inflammation.
Conclusion and Significance:
We introduce (19)F-MRI in combination with PFC nanoemulsions as a new platform to visualize abscess formation in a murine thigh infection model of S. aureus. The possibility to track immune cells in vivo by this modality offers new opportunities to investigate host immune response, the efficacy of antibacterial therapies and the influence of virulence factors for pathogenesis.
In der vorliegenden Arbeit wird der Einfluss von hs-CRP und Albumin auf die kardiovaskuläre Ereignisrate und das Überleben von Patienten mit Diabetes Mellitus Typ 2 an der Hämodialyse untersucht. Grundlagen für die hier dargestellte Auswertung sind die in der 4D Studie erhobenen Daten. Die 4D Studie hat prospektiv, randomisiert, doppelblind und placebo-kontrolliert untersucht, ob die Behandlung mit Atorvastatin bei Patienten mit Diabetes Mellitus Typ 2 an der Hämodialyse den primären Endpunkt bestehend aus Herzinfarkt, kardialem Tod und Schlaganfall zu senken vermag. Die Daten zum hs-CRP und Albumin wurden bei Studienbeginn und nach sechs Monaten erhoben. In einer post-hoc Analyse mit Hilfe eines multivariaten Cox Regressionsmodels konnte bestätigt werden, dass ein erhöhter Spiegel an hs-CRP und ein verminderter Spiegel an Albumin im Zusammenhang mit einer vermehrten kardiovaskulären Ereignisrate und Mortalität stehen. Eine Behandlung mit Atorvastatin führte zwar nicht zu einer Risikosenkung für den primären Endpunkt oder die Mortalität, hatte aber einen stabilisierenden Effekt auf des hs-CRP Spiegel.
Bei Patienten mit Diabetes mellitus Typ 2 und chronischer Niereninsuffizienz ist Anämie häufig. Zum Teil ist sie durch ungenügende EPO-Produktion bedingt. Zusätzlich wird die Hämoglobinsynthese, wie bei der Anämie chronischer Krankheiten (anemia of chronic disease, ACD) beschrieben, durch entzündliche Vorgänge unterdrückt. Der Stellenwert endogenen Erythropoietins bei Patienten mit diabetischer Nephropathie und ACD bleibt noch unsicher sowie auch der Zusammenhang zwischen EPO und der Nierenfunktion. Diese Querschnittsanalyse schloss 224 Patienten mit Typ 2-Diabetes in allen Stadien chronischer Niereninsuffizienz (CNI-Stadium 1-5) ein. Das mediane Alter betrug 67 Jahre, 54 % waren männlich und die mediane GFR lag bei 49 ml/min. Gemäß den Definitionen der K/DOQI-Richtlinien waren 41 % der Patienten anämisch. Von der Studie ausgeschlossen wurden wegen der Anämie behandelte Patienten und solche mit Eisenmangel. Prädiktoren der log-transformierten EPO-Spiegel wurden unter Benutzung multivariater linearer Regressionsmodelle ausgewertet. Die univariate und inverse Beziehung zwischen GFR und EPO-Spiegeln (p = 0,009) wurde in der multivariaten Analyse nicht-signifikant. Erhöhtes CRP (p < 0,001), niedriges Ferritin (p = 0,002), kardiovaskuläre Ereignisse in der Vorgeschichte (p = 0,02) und Hypertension (p = 0,04) waren nach Adjustierung für Alter, Geschlecht, Hb, GFR und andere klinische Faktoren unabhängig mit erhöhten EPO-Spiegeln assoziiert. In der untersuchten Population fand sich kein Zusammenhang zwischen EPO-Spiegeln und Hämoglobin. Bei diabetischen Patienten mit chronischer Niereninsuffizienz werden die EPO-Spiegel trotz gleichzeitig niedriger Hämoglobinspiegel vor allem durch Entzündungsparameter und den Eisenstatus vorhergesagt und sind dabei unabhängig von der Nierenfunktion. Deshalb könnte die Anämie bei Patienten mit diabetischer Nephropathie hauptsächlich durch Inflammation entstehen, die zu einem relativen Eisenmangel und einer Resistenz des Knochenmarks gegenüber endogenem EPO führt.
Pneumolysin, a protein toxin, represents one of the major virulence factors of Streptococcus pneumoniae. This pathogen causes bacterial meningitis with especially high disease rates in young children, elderly people and immunosuppressed patients. The protein toxin belongs to the family of cholesterol-dependent cytolysins, which require membrane cholesterol in order to bind and to be activated. Upon activation, monomers assemble in a circle and undergo conformational change. This conformational change leads to the formation of a pore, which eventually leads to cell lysis. This knowledge was obtained by studies that used a higher concentration compared to the concentration of pneumolysin found in the cerebrospinal fluid of meningitis patients. Thus, a much lower concentration of pneumolysin was used in this work in order to investigate effects of this toxin on primary mouse astrocytes. Previously, a small GTPase activation, possibly leading to cytoskeletal changes, was found in a human neuroblastoma cell line. This led to the hypothesis that pneumolysin can lead to similar cytoskeletal changes in primary cells. The aim of this work was to investigate and characterise the effects of pneumolysin on primary mouse astrocytes in terms of a possible pore formation, cellular trafficking and immunological responses. Firstly, the importance of pore-formation on cytoskeletal changes was to be investigated. In order to tackle this question, wild-type pneumolysin and two mutant variants were used. One variant was generated by exchanging one amino acid in the cholesterol recognising region, the second variant was generated by deleting two amino acids in a protein domain that is essential for oligomerisation. These variants should be incapable of forming a pore and were compared to the wild-type in terms of lytic capacities, membrane binding, membrane depolarisation, pore-formation in artificial membranes (planar lipid bilayer) and effects on the cytoskeleton. These investigations resulted in the finding that the pore-formation is required for inducing cell lysis, membrane depolarisation and cytoskeletal changes in astrocytes. The variants were not able to form a pore in planar lipid bilayer and did not cause cell lysis and membrane depolarisation. However, they bound to the cell membrane to the same extent as the wild-type toxin. Thus, the pore-formation, but not the membrane binding was the cause for these changes. Secondly, the effect of pneumolysin on cellular trafficking was investigated. Here, the variants showed no effect, but the wild-type led to an increase in overall endocytotic events and was itself internalised into the cell. In order to characterise a possible mechanism for internalisation, a GFP-tagged version of pneumolysin was used. Several fluorescence-labelled markers for different endocytotic pathways were used in a co-staining approach with pneumolysin. Furthermore, inhibitors for two key-players in classical endocytotic pathways, dynamin and myosin II, were used in order to investigate classical endocytotic pathways and their possible involvement in toxin internalisation. The second finding of this work is that pneumolysin is taken up into the cell via dynamin- and caveolin-independent pinocytosis, which could transfer the toxin to caveosomes. From there, the fate of the toxin remains unknown. Additionally, pneumolysin leads to an overall increase in endocytotic events. This observation led to the third aim of this work. If the toxin increases the overall rate of endocytosis, the question arises whether toxin internalisation favours bacterial tissue penetration of the host or whether it serves as a defence mechanism of the cell in order to degrade the protein. Thus, several proinflammatory cytokines were investigated, as previous studies describe an effect of pneumolysin on cytokine production. Surprisingly, only interleukin 6-production was increased after toxin-treatment and no effect of endocytotic inhibitors on the interleukin 6-production was observed. The conclusion from this finding is that pneumolysin leads to an increase of interleukin 6, which would not depend on the endocytotic uptake of pneumolysin. The production of interleukin 6 would enhance the production of acute phase proteins, T-cell activation, growth and differentiation. On the one hand, this activation could serve pathogen clearance from infected tissue. On the other hand, the production of interleukin 6 could promote a further penetration of pathogen into host tissue. This question should be further investigated.
Effekte von Paricalcitol auf Inflammation und Kalzifikationsregulation bei Hämodialysepatienten

Die Mortalität bei Dialysepatienten ist bedeutend höher als in der Allgemeinbevölkerung. Hauptgrund ist eine deutlich erhöhte kardiovaskuläre Morbidität und Mortalität. Als wichtige Ursachen hierfür gelten bei Dialysepatienten unter anderem das vermehrte Auftreten systemischer Inflammation und die Störung des Kalzium-Phosphathaushaltes, welche mit vermehrter vaskulärer Kalzifikation einhergeht. Da große Beobachtungsstudien darauf hinweisen, dass aktive Vitamin D-Therapie mit einem Überlebensvorteil für Dialysepatienten assoziiert ist, besteht die Hypothese, dass Paricalcitol antiinflammatorische und verkalkungsinhibitorische Effekte haben könnte.
In dieser vorliegenden multizentrischen, doppelverblindeten, prospektiven und Placebo-kontrollierten Crossover-Studie wurden 43 Hämodialysepatienten eingeschlossen, randomisiert und zwei Behandlungssequenzen zugeordnet. In der einen Behandlungssequenz erfolgte zunächst eine 12-wöchige Behandlung mit Paricalcitol (Startdosis 2 μg/Tag) und nach einer 4-wöchigen Washout- Phase eine 12-wöchige Behandlung mit Placebo. In der anderen Behandlungssequenz erfolgte nach gleichem Modus zunächst eine Behandlung mit Placebo und dann eine Behandlung mit Paricalcitol. Die Adjustierung der Dosis der Studienmedikation erfolgte entsprechend der Werte für Kalzium, Phosphat und PTH intakt. Zur Untersuchung der Hypothese wurden Zielparameter für Inflammation (hsCRP, Hepcidin) und Kalzifikation (Fetuin A , t-ucMGP, FGF-23) in regelmäßigen Intervallen gemessen. Als primärer Endpunkt wurden die 30%-ige Senkung des hsCRP-Levels und 20%-ige Steigerung der Fetuin A-Serumwerte definiert. Sekundäre Endpunkte waren Veränderungen der Serumkonzentrationen von Hepcidin, FGF-23 und t- ucMGP.
Insgesamt wurde die Studie von 25 Patienten protokollgemäß beendet. Bezüglich der primären Zielparameter zeigten sich keine signifikanten Unterschiede zwischen der Behandlung mit Paricalcitol und Placebo. Es konnte lediglich bei hsCRP ein leichter Trend zu niedrigeren Werten unter Paricalcitolbehandlung registriert werden. Bei den sekundären Zielparametern zeigte sich eine Borderline-Signifikanz (p = 0,051) hinsichtlich höherer FGF-23- Werte unter Paricalcitol. Bei Hepcidin und t-ucMGP konnten keine signifikanten Unterschiede zwischen der Behandlung mit Paricalcitol und Placebo verzeichnet werden.
In der vorliegenden Studie konnten die primären Endpunkte unter Paricalcitoltherapie somit nicht erreicht werden. Die Expression von Fetuin A wird von Paricalcitol wahrscheinlich nicht beeinflusst. Möglicherweise existiert aber ein leichter antiinflammatorischer Effekt, der eine Senkung der hsCRP- Serumwerte bedingen könnte. Des Weiteren steigert Paricalcitol die Expression von FGF-23, während die von Hepcidin und t-ucMGP unbeeinflusst zu sein scheint.
Durch Messung der Progranulinspiegel im Blutplasma mittels ELISA konnte in vorliegender Arbeit ein signifikanter Unterschied zwischen bipolaren Patienten und
Kontrollen nachgewiesen werden. Dabei wies das Patientenkollektiv durchschnittlich niedrigere Konzentrationen auf. Befunde vorausgegangener Studien deuten darauf hin, dass ein peripherer Progranulinmangel auch mit zentralnervösen Veränderungen einhergehen kann und somit einen Anteil an der Pathophysiologie der bipolaren Störung haben könnte. Insbesondere eine immunologisch-entzündliche Dysregulation sowie
fehlende neurotrophe Impulse könnten dabei eine wesentliche Rolle spielen.
Überdies fiel bei der Auswertung der Daten eine hochsignifikante Korrelation zwischen Progranulinspiegel und Lebensalter der Probanden auf. Dabei nahm mit steigendem Alter auch die periphere Progranulinkonzentration zu, was im Zuge des physiologischen Alterungsprozesses oder aber auch als Folge von neurodegenerativen bzw. -
inflammatorischen Vorgängen auftreten könnte.
Bei der molekulargenetischen Untersuchung der Polymorphismen rs2879096, rs4792938 und rs5848 konnten keine signifikanten Unterschiede in der Genotypen- und
Haplotypenverteilung zwischen Patienten und Kontrollen gefunden werden, was sich möglicherweise auf die relativ kleine Stichprobe von 181 Probanden zurückführen lässt.
Zumindest existieren Hinweise auf eine Rolle der jeweiligen Polymorphismen bei verschiedenen neuropsychiatrischen Erkrankungen, möglicherweise auch bei der bipolaren Störung.
Außerdem fand sich bei den im Progranulingen liegenden SNPs rs2879096 und rs4792938 keine Assoziation einer Risikogenvariante zu veränderten Progranulinspiegeln, das heißt keiner der beiden Polymorphismen scheint funktionelle Bedeutung zu haben. Somit ließe sich kein Effekt des Genotyps auf die periphere Progranulinkonzentration nachweisen, wobei wiederum die verhältnismäßig niedrigen Fallzahlen beachtet werden müssen.
Der in der 3’UTR liegende SNP rs5848 scheint hingegen im Patientenkollektiv zu deutlich erniedrigten Proteinspiegeln zu führen. Dieser Befund steht auch im Einklang 47 mit einer Vielzahl von Studien, die rs5848 eine Bedeutung bei verschiedenen neuropsychiatrischen Erkrankungen beimessen und unterstreicht die Rolle niedriger Progranulinkonzentrationen in der bipolaren Störung.
Zusammenfassend lässt sich feststellen, dass bei bipolaren Patienten signifikant niedrigere Progranulinkonzentrationen als in der Kontrollgruppe gemessen wurden.
Weitere Studien, welche die zu Grunde liegenden biochemischen Vorgänge und Pathomechanismen erforschen, wären nun von Interesse, um ein besseres Verständnis
der Erkrankung zu erlangen. Langfristig wäre die Nutzung des Progranulinspiegels als diagnostisches Werkzeug erstrebenswert, wobei sich dies auf Grund des großen
Overlaps zwischen Patienten- und Kontrollgruppe wohl eher schwierig gestalten wird.
Die relativ unkomplizierte Progranulinbestimmung im Blutplasma könnte möglicherweise im Rahmen einer Messung vieler verschiedener Parameter als Baustein eines diagnostischen Apparates zur Erkennung der bipolaren affektiven Störung dienen. Somit wären weiterführende Untersuchungen anhand größerer Fallzahlen und
funktionelle Studien der pathomechanistischen Zusammenhänge des Progranulins ein interessantes Feld, um einige der zahlreichen ungeklärten Fragen rund um die bipolare Störung weiter aufzuklären.
Background
The role of the immune system in the pathophysiology of acute ischemic stroke is increasingly recognized. However, targeted treatment strategies to modulate immunological pathways in stroke are still lacking. Glatiramer acetate is a multifaceted immunomodulator approved for the treatment of relapsing-remitting multiple sclerosis. Experimental studies suggest that glatiramer acetate might also work in other neuroinflammatory or neurodegenerative diseases apart from multiple sclerosis.
Findings
We evaluated the efficacy of glatiramer acetate in a mouse model of brain ischemia/reperfusion injury. 60 min of transient middle cerebral artery occlusion was induced in male C57Bl/6 mice. Pretreatment with glatiramer acetate (3.5 mg/kg bodyweight) 30 min before the induction of stroke did not reduce lesion volumes or improve functional outcome on day 1.
Conclusions
Glatiramer acetate failed to protect from acute ischemic stroke in our hands. Further studies are needed to assess the true therapeutic potential of glatiramer acetate and related immunomodulators in brain ischemia.
