In dieser Studie wurden 280 Patienten, die im Zeitraum vom 01.01.2008 bis
30.06.2012 am Universitätsklinikum Würzburg aufgrund einer Raumforderung der
Leber operiert wurden, analysiert. Davon konnten 150 Patienten mit insgesamt 385
Tumoren bewertet werden. Die zentrale Fragestellung der Arbeit beschäftigte sich
damit, die Sensitivität der MR-DWI mit anderen präoperativen bildgebenden
Untersuchungsmethoden zu bewerten. Bisherige Studien beschäftigten sich bereits
mit dem Vergleich der MR-DWI mit anderen diagnostischen Verfahren. Jedoch
wurden dabei nicht konkret die aus Sicht eines Chirurgen wichtigen Punkte detailliert
beleuchtet. In dieser Arbeit wurden daher die Ergebnisse der Befunde der MR-DWI,
CT und konventionellem MRT dem intraoperativen und histopathologisch
gesichertem Endbefund gegenübergestellt und bezüglich der Parameter:
Tumordignität, Tumoranzahl, Tumorgröße und Tumorlokalisation verglichen.
Während die Sensitivität der MR-DWI zur Tumorlokalisation etwas unter der
Sensitivität der CT und MRT lag, konnten für die Parameter Tumoranzahl und
Tumordignität bessere Ergebnisse der MR-DWI als für die CT herausgestellt werden.
Die Genauigkeit der DWI für diese beiden Punkte liegt nah bei der hohen Sensitivität
der MRT. Jedoch müssen die sehr guten Ergebnisse der MRT in dieser Arbeit durch
die geringe Anzahl der MRT Untersuchungen relativiert werden.
Bezüglich der Tumoranzahl ergibt sich bei der MR-DWI die höchste Sensitivität,
gefolgt von MRT und CT.
Zusammenfassend kann gesagt werden, dass die MR-DWI wie auch die CT und
MRT ein sehr sensitives Verfahren zur Detektion von Leberläsionen darstellt. Dabei
müssen gewisse Vor- und Nachteile gegenüber den anderen Methoden abgewogen
werden. Eine als ungünstig zu wertende Strahlenexposition besteht nicht, ebenso
kein zusätzlicher zeitlicher Aufwand.
In nahezu allen hier untersuchten Bereichen zeigte die MR-DWI vorrangig bessere
Resultate als die CT. Im Vergleich mit der MRT konnten geringfügig niedrigere oder
ähnlich gute Werte ermittelt werden. In Bezug auf die Tumoranzahl war die MR-DWI
das sensitivste Verfahren.
Um die hier erbrachten Ergebnisse zu sichern, sollten zukünftige Studien die
Homogenität der Patienten sowie die Anzahl der CT, MRT und MR-DWI
Untersuchungen erhöhen. Weiterhin könnten die Geräteeinstellungen bei der MRDWI
an bestimmte Fragestellungen angepasst und so genauere Ergebnisse erzielt
werden. Bruegel et al konnte für unterschiedliche Leberläsionen bereits die
charakteristischen ADC-Werte ermitteln. In einer weiteren Studie von
Kilickesmez wurden ebenfalls maligne und benigne Leberläsionen über die
entsprechenden ADC-Werte voneinander abgegrenzt. Daraus ergab sich die
Empfehlung, zur Diagnostik von Leberläsionen die konventionelle MRT mit der MRDWI
zu ergänzen.
Ein weiterer Ansatzpunkt ist die Reduktion von Störfaktoren für die MR-DWI und
damit die Verbesserung der Bildbeurteilung. In einer Arbeit von Yu-Chun wurde
beispielsweise die optimale Temperatur für die MR-DWI Schnittbildgebung am
Tiermodell angewandt und so auf molekularer Ebene die Artefaktbildung reduziert.
Eine Verbesserung konnte auch durch Ergänzung der MR-DWI mit anderen
Verfahren nachgewiesen werden. MR-DWI in Verbindung mit Kontrastmittel ergab
eine erhöhte Sensitivität für kleine HCC bis 2cm als Kontrastmittel-MRT ohne DWI.
Es ist zu erwarten, dass die MR-DWI in Zukunft durch exaktere Einstellungen der b-
Werte und ADC-Messungen in Abhängigkeit der zu untersuchenden Organregion,
durch Reduktion störender Faktoren oder durch Kombination mit anderen
bildgebenden Verfahren zur Optimierung der präoperativen Diagnostik bei
Lebertumoren beiträgt.
Ziel dieser Arbeit war es, die Auswirkungen der Änderungen der Therapiestandards in der Behandlung des Kolonkarzinoms und die Auswirkungen der Einführung der Vorsorgekoloskopie auf die Überlebensraten der Patienten mit Kolonkarzinom zu untersuchen.
Die umfassende Analyse der therapieabhängigen Überlebensraten von 1016 Patienten mit Kolonkarzinom aus 20 Jahren zeigt eine Verbesserung der Überlebenswahrscheinlichkeit durch den Einsatz adjuvanter Therapie und multimodaler Therapieregime. Durch Neuerungen in der Therapie konnten die 5-Jahres-Überlebensraten seit Anfang der 90er Jahre nahezu verdoppelt werden. Als wichtigste Prädiktoren für das Langzeitüberleben stellten sich das Alter der Patienten bei Erstdiagnose, das UICC Stadium und die Art der adjuvanten Therapie heraus. Der Überlebenszeit verlängernde Effekt war für den Einsatz der heutigen Standardtherapie mit 5-Flourouracil (5-FU) schon signifikant und zeigt sich für die Kombination mit neueren Medikamenten, insbesondere Oxaliplatin, noch deutlicher. Neue Operationstechniken, Fortschritte in der Metastasenchirurgie, ein optimiertes supportives Management und weitere Erkenntnisse onkologischer Prinzipien beeinflussten die erzielten Erfolge synergistisch.
Das Gesamtüberleben der Patienten, die per Vorsorgekoloskopie detektiert werden ist besser als das der Patienten, die aufgrund klinischer Symptome diagnostiziert werden. Neben dem signifikanten Überlebensvorteil der Früherkennungs-Patienten, der sich durch die niedrigeren UICC Stadien in dieser Gruppe ergibt, finden sich auch Trends bezüglich eines besseren Outcomes dieser Patienten innerhalb der selben UICC Stadien. Die Patienten, deren Tumor im Rahmen des Screenings detektiert wurde, waren signifikant jünger, wiesen signifikant weniger Begleiterkrakungen auf und zeigten signifikant niedrigere Tumorstadien. Eine adjuvante Therapie wurde in der Screening-Gruppe signifikant häufiger durchgeführt. Mehr als einer von fünf tumorbedingten Todesfällen der Patienten, die augrund von Symptomen diagnostiziert wurden, hätte in dieser Studienpopulation verhindert werden können, wenn eine routinemäßige Vorsorgekoloskopie durchgeführt worden wäre.
Das Fazit lautet: die Vorsorgekoloskopie ist effektiv. Die Tumorgenese kann durch Entfernung von Voräuferläsionen durchbrochen werden, Tumoren können in frühen asymptomatischen Stadien detektiert werden. Screeningprogramme sollten erweitert werden, um die Inzidenz und die Mortalität von Darmkrebs weiter zu senken.
Hier wird über den objektiven Vergleich der Kurz- und Langzeitergebnisse, die mit der laparoskopischen Heller-Myotomie plus Dor-Fundoplikatio (LHM + Dor) und der per-oralen endoskopischen Myotomie (POEM) an einer einzigen Institution erzielt wurden und über die Entwicklung eines "maßgeschneiderten Ansatzes" berichtet. Die laparosko-pischen Heller Myotomie (LHM) gilt aktuell als das Standardverfahren zur Therapie der Achalasie. Die perorale endoskopische Myotomie (POEM) bietet eine neue Alternative zur Durchführung der Heller-Myotomie auf endoskopischem und natural orifice translu-minal endoscopic surgery (NOTES) Wege.
Wir berichten über unsere unselektierte konsekutive Patientenserien (n = 238), die zwi-schen 2009 und 2017 entweder einer LHM + Dor Prozedur (n = 100) oder einer POEM Operation (n = 138) unterzogen wurden.
Es gab keine Unterschiede hinsichtlich des Behandlungserfolgs (Eckardt-Score ≤ 3) nach 3 - 6 Monaten (LHM: 90,1% vs. POEM: 90,5%; p = 0,735), 12 Monate (LHM 84,4% vs. POEM 84,0%; p = 0,766 und> 2 Jahre (LHM 81,1% vs. POEM 79,2%; p = 0,432). Die Verbesserung der Lebensqualität (GIQLI-Score) und die Patientenzufriedenheit waren in beiden Gruppen ebenfalls ver-gleichbar gut (~ 90%) und zu jedem Zeitpunkt der Nachbeobachtung. Refluxraten sind ebenfalls ähnlich (Sodbrennen LHM: 32,9% vs. POEM: 30,1%; Ösophagitis LHM: 16,3% vs. POEM: 14,6%; PPI-Aufnahme 21,2% vs. 32,5%, pathologischer pH-Wert -Meter-LHM: 8,8% vs. POEM: 23,3%) ohne Signifikanz der Unterschiede.
Die „maßgeschneiderte Verfahrenswahl POEM vs. LHM“ könnte geeignet sein und sollte in weiteren Studien weiter untersucht werden.
TNF-like weak inducer of apoptosis (TWEAK) and inhibition of protein synthesis with cycloheximide (CHX) sensitize for poly(I:C)-induced cell death. Notably, although CHX preferentially enhanced poly(I:C)-induced apoptosis, TWEAK enhanced primarily poly(I:C)-induced necroptosis. Both sensitizers of poly(I:C)-induced cell death, however, showed no major effect on proinflammatory poly(I:C) signaling. Analysis of a panel of HeLa-RIPK3 variants lacking TRADD, RIPK1, FADD, or caspase-8 expression revealed furthermore similarities and differences in the way how poly(I:C)/TWEAK, TNF, and TRAIL utilize these molecules for signaling. RIPK1 turned out to be essential for poly(I:C)/TWEAK-induced caspase-8-mediated apoptosis but was dispensable for this response in TNF and TRAIL signaling. TRADD-RIPK1-double deficiency differentially affected poly(I:C)-triggered gene induction but abrogated gene induction by TNF completely. FADD deficiency abrogated TRAIL- but not TNF- and poly(I:C)-induced necroptosis, whereas TRADD elicited protective activity against all three death inducers. A general protective activity against poly(I:C)-, TRAIL-, and TNF-induced cell death was also observed in FLIPL and FLIPS transfectrants.
Background
International consensus criteria (ICC) have redefined borderline resectability for pancreatic ductal adenocarcinoma (PDAC) according to three dimensions: anatomical (BR-A), biological (BR-B), and conditional (BR-C). The present definition acknowledges that resectability is not just about the anatomic relationship between the tumour and vessels but that biological and conditional dimensions also are important.
Methods
Patients’ tumours were retrospectively defined borderline resectable according to ICC. The study cohort was grouped into either BR-A or BR-B and compared with patients considered primarily resectable (R). Differences in postoperative complications, pathological reports, overall (OS), and disease-free survival were assessed.
Results
A total of 345 patients underwent resection for PDAC. By applying ICC in routine preoperative assessment, 30 patients were classified as stage BR-A and 62 patients as stage BR-B. In total, 253 patients were considered R. The cohort did not contain BR-C patients. No differences in postoperative complications were detected. Median OS was significantly shorter in BR-A (15 months) and BR-B (12 months) compared with R (20 months) patients (BR-A vs. R: p = 0.09 and BR-B vs. R: p < 0.001). CA19-9, as the determining factor of BR-B patients, turned out to be an independent prognostic risk factor for OS.
Conclusions
Preoperative staging defining surgical resectability in PDAC according to ICC is crucial for patient survival. Patients with PDAC BR-B should be considered for multimodal neoadjuvant therapy even if considered anatomically resectable.
Background
Pancreatic adenocarcinoma (PDAC) patients with preoperative carbohydrate antigen 19-9 (CA19-9) serum levels higher than 500 U/ml are classified as biologically borderline resectable (BR-B). To date, the impact of cholestasis on preoperative CA19-9 serum levels in these patients has remained unquantified.
Methods
Data on 3079 oncologic pancreatic resections due to PDAC that were prospectively acquired by the German Study, Documentation and Quality (StuDoQ) registry were analyzed in relation to preoperative CA19-9 and bilirubin serum values. Preoperative CA19-9 values were adjusted according to the results of a multivariable linear regression analysis of pathologic parameters, bilirubin, and CA19-9 values.
Results
Of 1703 PDAC patients with tumor located in the pancreatic head, 420 (24.5 %) presented with a preoperative CA19-9 level higher than 500 U/ml. Although receiver operating characteristics (ROC) analysis failed to determine exact CA19-9 cut-off values for prognostic indicators (R and N status), the T, N, and G status; the UICC stage; and the number of simultaneous vein resections increased with the level of preoperative CA19-9, independently of concurrent cholestasis. After adjustment of preoperative CA19-9 values, 18.5 % of patients initially staged as BR-B showed CA19-9 values below 500 U/ml. However, the postoperative pathologic results for these patients did not change compared with the patients who had CA19-9 levels higher than 500 U/ml after bilirubin adjustment.
Conclusions
In this multicenter dataset of PDAC patients, elevation of preoperative CA19-9 correlated with well-defined prognostic pathologic parameters. Bilirubin adjustment of CA19-9 is feasible but does not affect the prognostic value of CA19-9 in jaundiced patients.
Es wurden 6 verschiedene Brustimplantate mit unterschiedlichen Füllmaterialien auf ihre Strahlendurchlässigkeit geprüft. Ein Implantat mit dem Füllmaterial Sojaöl, zwei mit Silikon, davon eins mit Polyurethanbeschichtung und zwei mit PVP- Hydrogel gefüllt. Zur Simulation von in vivo - Bedingungen wurde weibliches Brustgewebe, welches bei Mammareduktionen gewonnen wurde, verwendet. Außerdem ein Phantom welches Tumorgewebe, Mikrokalk und fibröse Veränderungen imitieren sollte. Nach Durchführung der Mammographien mit 16 verschiedenen Gewebetaschen wurden die optischen Dicheten für die einzelnen Implantate mit einem Densitometer ermittelt (objektiv quantitative Auswertung) und desweiteren fünf Radiologen in einem Doppelblindverfahren vorgelegt (subjektiv qualitative Auswertung). Es zeigt sich signifikant höhere Strahlendurchlässigkeit für Sojaöl und PVP als Füllmaterial.
Auch nach jahrelanger Forschung ist eine Transplantation von Langerhans-Inseln noch keine Standard-Therapie des Insulin-abhängigen Diabetes mellitus. Ein Problem ist die grosse Anzahl benötigter Pankreata, mögliche Lösung die Verwendung porziner Langerhans-Inseln. Damit die späteren Spender-Langerhans-Inseln ein gutes In-Vivo-Ergebnis erzielen können, muss bereits eine gute Funktion in vitro gewährleistet sein. In dieser Arbeit wurde untersucht, ob sich eine verbesserte Funktion der Langerhans-Inseln durch eine Exposition von extrazellulären Matrixproteinen (ECMP) erreichen lässt. Weiterhin wurde ein Einfluss von Polyelektrolytschichten auf die Funktion der Langerhans-Inseln und ihre mikroskopische Darstellbarkeit untersucht. Mit der sogenannten Nanoverkapselung soll bei der Transplantation eine Immunisolation der Spender-Inseln als Schutz vor dem Immunsystem des Empfängers erreicht werden. Es wurden zwei verschiedene Kapseln untersucht: Chitosan MMW/PEGPPGPEG/Chitosan MMW (CPC) und Chitosan MMW/PAAMA/PAH 15 kDa (CPP). Wir konnten zeigen, dass die Funktion der Langerhans-Inseln generell am Tag +6 besser ist als am Tag +1, weil erst dann eine Erholung vom Stress der Isolierung stattgefunden hat. Eine Verbesserung der Inselzell-Funktion durch die ECMP konnte bestätigt werden, allerdings hängt sie von den jeweils verwendeten Komponenten der extrazellulären Matrix ab. Durchweg gute Perifusionsergebnisse am Tag +6 werden nach Exposition mit Kollagen IV, Laminin und Fibronektin erreicht. Bei den Versuchen mit den Polyelektrolyt-Nanokapseln ergaben sich deutliche Unterschiede zwischen den beiden untersuchten Nanokapseln. Bei allen verwendeten Polyelektrolyten war auf gute Biokompatibilität geachtet worden. Dennoch scheinen die Schichten CPC einen negativen Einfluss auf die Langerhans-Inseln zu haben: Hier kam es zu einer verschlechterten Funktion der Langerhans-Inseln am Tag +6. Die Langerhans-Inseln mit der Nanokapsel CPP dagegen zeigten gute Ergebnisse. Interessanterweise sahen wir aber bei CPP-Nanokapseln eine verstärkte Autofluoreszenz, ein Zeichen für zellulären Stress. Durch die Verwendung von FITC-gekoppelten Polyelektrolyten lässt sich mikroskopisch eine verstärkte Fluoreszenz der Langerhans-Inseln nachweisen. Der Nachweis der Vollständigkeit der Polyelektrolytschichten muss jedoch in weiteren Studien erbracht werden. Die hier gewonnenen Erkenntnisse über die extrazellulären Matrixproteine sind zunächst für zukünftige Versuche wichtig: Die Funktion der Langerhans-Inseln kann so verbessert werden, dies ist eine Voraussetzung für erfolgreiche Versuche. Ausserdem können die ECMP für die spätere Vorbereitung von Langerhans-Inseln vor der Transplantation verwendet werden. Entscheidend ist, dass die Langerhans-Inseln für mehrere Tage mit den ECMP inkubiert werden, um so ihre Funktion zu verbessern. Bei der Polyelektrolyt-Nanoverkapselung besteht noch viel Forschungsbedarf: Anhand des Erfolg versprechenden Ergebnisses mit der CPP-Nanokapsel muss weitergearbeitet werden. Offensichtlich führt – bei Auswahl der richtigen Polyelektrolyte – die Nanoverkapselung nicht zu einer verschlechterten Funktion der Langerhans-Inseln. Bei einer grossen Anzahl in Frage kommender Polyelektrolyte muss die optimale Kombination von Schichten gefunden und ihre Fähigkeit zur Immunisolation untersucht werden. Bezüglich der Darstellung der Nanokapseln bietet sich die konfokale Fluoreszenzmikroskopie an.
Ischemia-like stress conditions stimulate trophic activities of adipose-derived stromal/stem cells

Adipose-derived stromal/stem cells (ASCs) have been shown to exert regenerative functions, which are mainly attributed to the secretion of trophic factors. Upon transplantation, ASCs are facing an ischemic environment characterized by oxygen and nutrient deprivation. However, current knowledge on the secretion capacity of ASCs under such conditions is limited. Thus, the present study focused on the secretory function of ASCs under glucose and oxygen deprivation as major components of ischemia. After exposure to glucose/oxygen deprivation, ASCs maintained distinct viability, but the metabolic activity was greatly reduced by glucose limitation. ASCs were able to secrete a broad panel of factors under glucose/oxygen deprivation as revealed by a cytokine antibody array. Quantification of selected factors by ELISA demonstrated that glucose deprivation in combination with hypoxia led to markedly higher secretion levels of the angiogenic and anti-apoptotic factors IL-6, VEGF, and stanniocalcin-1 as compared to the hypoxic condition alone. A conditioned medium of glucose/oxygen-deprived ASCs promoted the viability and tube formation of endothelial cells, and the proliferation and migration of fibroblasts. These findings indicate that ASCs are stimulated by ischemia-like stress conditions to secrete trophic factors and would be able to exert their beneficial function in an ischemic environment.
In vorliegender Dissertation wurde zunächst die Häufigkeit einer trampolinassoziierten Verletzung bei Kindern aus einem Patientenkollektiv der Abteilung für Kinderchirurgie der Universität Würzburg ermittelt. Dabei sollten alters- und geschlechtsspezifische Unterschiede, sowie der Unfallhergang Berücksichtigung finden. In einer ausführlichen Darstellung wurden die zugezogenen Verletzungen, sowie deren Therapie veranschaulicht. Zuletzt wurde in einer Gesamtschau ermittelt, ob die Anzahl der trampolinassoziierten Verletzungen in den letzten 7 Jahren in Würzburg zugenommen hat.
