Background
Increasing bacterial resistance to antibiotics is a serious problem worldwide. We sought to record the acquisition of antibiotic-resistant Escherichia coli (E. coli) in healthy infants in Northern Thailand and investigated potential determinants.
Methods
Stool samples from 142 infants after birth, at ages 2wk, 2mo, 4 to 6mo, and 1y, and parent stool samples were screened for E. coli resistance to tetracycline, ampicillin, co-trimoxazole, and cefazoline by culture, and isolates were further investigated for multiresistance by disc diffusion method. Pulsed-field gel electrophoresis was performed to identify persistent and transmitted strains. Genetic comparison of resistant and transmitted strains was done by multilocus sequence typing (MLST) and strains were further investigated for extra- and intra-intestinal virulence factors by multiplex PCR.
Results
Forty-seven (33%) neonatal meconium samples contained resistant E. coli. Prevalence increased continuously: After 1y, resistance proportion (tetracycline 80%, ampicillin 72%, co-trimoxazole 66%, cefazoline 35%) almost matched those in parents. In 8 infants (6%), identical E. coli strains were found in at least 3 sampling time points (suggesting persistence). Transmission of resistant E. coli from parents to child was observed in only 8 families. MLST showed high diversity. We could not identify any virulence genes or factors associated with persistence, or transmission of resistant E. coli. Full-term, vaginal birth and birth in rural hospital were identified as risk factors for early childhood colonization with resistant E. coli.
Conclusion
One third of healthy Thai neonates harboured antibiotic-resistant E. coli in meconium. The proportion of resistant E. coli increased during the first year of life almost reaching the value in adults. We hypothesize that enhancement of infection control measures and cautious use of antibiotics may help to control further increase of resistance.
In der Scheidungsforschung werden Kinder als ein zentraler Einflussfaktor der Ehestabilität thematisiert. Je nachdem welches Kindschaftsverhältnis oder Charakteristikum von Kindern vorliegt, ist mit der Erhöhung oder Verminderung der Ehestabilität zu rechnen. Zum Beispiel wird nach der Familienökononmie, durch das Vorhandensein von unehelichen Kindern in einer Ehe die Ehestabilität vermindert, während eheliche Kinder das Scheidungsrisiko reduzieren. In den meisten Untersuchungen der Scheidungsforschung werden die unterschiedlichen Einflüsse von Elternschaft auf das Scheidungsrisiko dennoch weniger gut beleuchtet. Weiterhin ist in Anlehnung an die Theorien davon auszugehen, dass der Einfluss von Kindern auf die Ehestabilität nur erforscht werden kann, wenn bei der Operationalisierung der Kindvariablen eindeutig nach Kindschaftsverhältnissen differenziert wird, dieses Vorgehen findet sich allerdings nur in wenigen Studien der Scheidungsforschung. Diese Dissertation versucht die Lücke zu schließen, indem sie die Einflüsse unterschiedlicher Charakteristika von Kindern und verschiedener Kindschaftsverhältnisse auf die Ehestabilität mit den Daten des Familiensurveys 2000 methodisch korrekt und in einem möglichst großem Spektrum untersucht. In dieser Arbeit werden die unterschiedlichen Kindschaftsverhältnisse und Charakteristika von Kindern so operationalisiert, dass der Einfluss anderer Effekte ausgeschlossen wird. Nur durch dieses Vorgehen können letztendlich die Einflüsse von Kindern auf die Ehestabilität eindeutig aufgezeigt werden. Die Ergebnisse der Dissertation zeigen zum einen, wie die relevanten Theorien den Einfluss von Kindern auf die Ehestabilität begründen und zum anderen bestätigen die Befunde der multivariaten Analysen, dass in Abhängigkeit von Kindschaftsverhältnis oder Charakteristika der Kinder unterschiedliche Richtungen des Scheidungsrisikos zu berichten sind. Dadurch kann die Vermutung bestätigt werden, dass die exakte Differenzierung nach Kindschaftsverhältnissen und unterschiedlichen Charakteristika von Kindern und die methodisch korrekte Operationalisierung der Kindvariablen bedeutsame Unterschiede in den Scheidungsraten aufdeckt.
HINTERGRUND: Eltern spielen im Rahmen der postoperativen Schmerztherapie von Kindern eine immer wichtigere Rolle. Validierte und einfach anzuwendende Instrumente zur Fremdbeobachtung durch Eltern können die Schmerzmessung verbessern. Ziel der Studie war daher die Evaluation einer deutschen Version der englischsprachigen Parents' Postoperative Pain Measure (PPPM-D). ERGEBNISSE: Die Relabilität zeigte ausreichend gute Cronbach's-alpha-Werte. Hohe Korrelationen mit den Vergleichsinstrumenten fanden sich als Ausdruck der KOntruktvalidität. Die diskriminative Validität zeigte an den ersten 3 postoperativen Tagen klinisch relevante und statistisch signifikante Unterschiede zwischen den nach Schwerefrad der Operation eingeteilten Gruppen. Zudem fanden sich Hinweise auf gute Änderungssensitivität. Die PPPM-D wurde als gut anwendbar und von der Mehrheit der Eltern als wichtig bewertet. SCHLUSSFOLGERUNG: Die PPPM-D ist ein von Eltern akzeptiertes, valides und reliables Instrument zur Fremdbeurteilung postoperativer Schmerzen bei Kindern zwischen dem 2. und 12. Lebensjahr nach orthopädischen Operationen.
Treatment of dysferlinopathy with deflazacort: a double-blind, placebo-controlled clinical trial

Background: Dysferlinopathies are autosomal recessive disorders caused by mutations in the dysferlin (DYSF) gene encoding the dysferlin protein. DYSF mutations lead to a wide range of muscular phenotypes, with the most prominent being Miyoshi myopathy (MM) and limb girdle muscular dystrophy type 2B (LGMD2B).
Methods: We assessed the one-year-natural course of dysferlinopathy, and the safety and efficacy of deflazacort treatment in a double-blind, placebo-controlled cross-over trial. After one year of natural course without intervention, 25 patients with genetically defined dysferlinopathy were randomized to receive deflazacort and placebo for six months each (1 mg/kg/day in month one, 1 mg/kg every 2nd day during months two to six) in one of two treatment sequences. Results: During one year of natural course, muscle strength declined about 2% as measured by CIDD (Clinical Investigation of Duchenne Dystrophy) score, and 76 Newton as measured by hand-held dynamometry. Deflazacort did not improve muscle strength. In contrast, there is a trend of worsening muscle strength under deflazacort treatment, which recovers after discontinuation of the study drug. During deflazacort treatment, patients showed a broad spectrum of steroid side effects.
Conclusion: Deflazacort is not an effective therapy for dysferlinopathies, and off-label use is not warranted. This is an important finding, since steroid treatment should not be administered in patients with dysferlinopathy, who may be often misdiagnosed as polymyositis.
Tiaprid wird bei Kindern und Jugendlichen im deutschsprachigen Raum als Mittel der ersten Wahl zur Behandlung von Ticstörungen off-label eingesetzt. Es gilt dabei die generelle Empfehlung, Therapeutischen Drug Monitoring (TDM) bei der Behandlung von Minderjährigen mit Neuro-/Psychopharmaka durchzuführen. Therapeutische Referenzbereiche für Tiaprid sind bisher jedoch nur für erwachsene Patienten mit Chorea Huntington definiert worden (1000 bis 2000 ng/ml) (Hiemke et al., 2011).
An ausgewählten Zentren im Rahmen des Kompetenznetzwerks Therapeutisches Drug Monitoring Kinder- und Jugendpsychiatrie (www.tdm-kjp.com) wurden von 2007 bis 2014 standardisiert TDM-Daten erfasst, um den Zusammenhang zwischen Dosis, Serumkonzentration, Wirksamkeit und UAW von Tiaprid zu untersuchen sowie Hinweise auf einen möglichen alters- und diagnosespezifischen therapeutischen Referenzbereich zu generieren.
Bei den 49 Patienten (mittleres Alter 12,5 Jahre; 84 % männlich) zeigte sich eine positive Korrelation (r= 0.76; p< .001) zwischen der Dosis (Mittelwert 354 mg) und der Serumkonzentration von Tiaprid (Mittelwert 1324 ng/ml) mit einer ausgeprägten interindividuellen Variabilität, jedoch keine Beziehung zwischen Serumkonzentration und Wirkeffekt (83,3 % profitierten) bzw. UAW in der Gesamtpopulation.
Die Auswertung der Verlaufsmessungen von Patienten mit mehreren Messungen der Tiaprid-Serumkonzentration ergab beim dritten Messzeitpunkt eine negative Korrelation zwischen Wirkeffekt und Serumkonzentration (r= -.68; p= .032). Bei Patienten mit Serumkonzentrationen unter 2000 ng/ml wurde ein günstigerer klinischer Effekt dokumentiert als bei solchen mit Konzentrationen oberhalb dieses Wertes. Die ROC-Analyse ergab eine Sensitivität von 86 %, ab einer Konzentration von 618 ng/ml zu respondieren (AUC= .524). Kein Patient litt an einer schweren UAW und nur wenige Patienten unter leichten oder mittelschweren UAW (n=13).
Diese Ergebnisse lassen vermuten, dass der untere therapeutische Referenzbereich für jugendliche Patienten mit einer Tic-Störung bei etwa 600 ng/ml liegt und die obere Grenze von 2000 ng/ml auch als Orientierungswert auf Kinder und Jugendliche gelten könnte. Bevor diesbezüglich gültige Empfehlungen für den klinischen Alltag formuliert werden, müssen Studien mit höheren Fallzahlen und mehr kontrollierten Studiendesigns abgewartet werden.
Risperidone is commonly used to treat different psychiatric disorders worldwide. Knowledge on dose–concentration relationships of risperidone treatment in children and adolescents with schizophrenia or other psychotic disorders is, however, scarce and no age-specific therapeutic ranges have been established yet. Multicenter data of a therapeutic drug monitoring service were analyzed to evaluate the relationship between risperidone dose and serum concentration of the active moiety (risperidone (RIS) plus its main metabolite 9-hydroxyrisperidone (9-OH-RIS)) in children and adolescents with psychotic disorders. Patient characteristics, doses, serum concentrations and therapeutic outcomes were assessed by standardized measures. The study also aimed to evaluate whether the therapeutic reference range for adults (20–60 ng/ml) is applicable for minors. In the 64 patients (aged 11–18 years) included, a positive correlation between daily dose and the active moiety (RIS\(_{am}\)) concentration was found (r\(_s\) = 0.49, p = 0.001) with variation in dose explaining 24% (r\(_s\)\(^2\) = 0.240) of the variability in serum concentrations. While the RIS\(_{am}\) concentration showed no difference, RIS as well 9-OH-RIS concentrations and the parent to metabolite ratio varied significantly in patients with co-medication of a CYP2D6 inhibitor. Patients with extrapyramidal symptoms (EPS) had on average higher RIS\(_{am}\) concentrations than patients without (p = 0.05). Considering EPS, the upper threshold of the therapeutic range of RIS\(_{am}\) was determined to be 33 ng/ml. A rough estimation method also indicated a possibly decreased lower limit of the preliminary therapeutic range in minors compared to adults. These preliminary data may contribute to the definition of a therapeutic window in children and adolescents with schizophrenic disorders treated with risperidone. TDM is recommended in this vulnerable population to prevent concentration-related adverse drug reactions.
Background: Routine annual influenza vaccination is primarily recommended for all persons aged 60 and above and for people with underlying chronic conditions in Germany. Other countries have already adopted additional childhood influenza immunisation programmes. The objective of this study is to determine the potential epidemiological impact of implementing paediatric influenza vaccination using intranasally administered live-attenuated influenza vaccine (LAIV) in Germany.
Methods: A deterministic age-structured model is used to simulate the population-level impact of different vaccination strategies on the transmission dynamics of seasonal influenza in Germany. In our base-case analysis, we estimate the effects of adding a LAIV-based immunisation programme targeting children 2 to 17 years of age to the existing influenza vaccination policy. The data used in the model is based on published evidence complemented by expert opinion.
Results: In our model, additional vaccination of children 2 to 17 years of age with LAIV leads to the prevention of 23.9 million influenza infections and nearly 16 million symptomatic influenza cases within 10 years. This reduction in burden of disease is not restricted to children. About one third of all adult cases can indirectly be prevented by LAIV immunisation of children.
Conclusions: Our results demonstrate that vaccinating children 2-17 years of age is likely associated with a significant reduction in the burden of paediatric influenza. Furthermore, annual routine childhood vaccination against seasonal influenza is expected to decrease the incidence of influenza among adults and older people due to indirect effects of herd protection. In summary, our model provides data supporting the introduction of a paediatric influenza immunisation programme in Germany.
Objectives: The purpose of this study was to investigate the development of lateralization skills in children who received bilateral cochlear implants (CIs) in sequential operations. Methods: The lateralization skills of 9 children with a mean age of 4.1 years at the first surgery and 5.5 years at the second surgery were assessed at 3 time intervals. Children were assessed with a 3-loudspeaker setup (front, left and right) at 0.9 years (interval I) and 1.6 years (interval II) after the second implantation, and after 5.3 years of bilateral implant use (interval III) with a 9-loudspeaker setup in the frontal horizontal plane between -90° and 90° azimuth. Results: With bilateral implants, a significant decrease in lateralization error was noted between test interval I (45.0°) and II (23.3°), with a subsequent significant decrease at test interval III (4.7°). Unilateral performance with the CI did not improve significantly between the first 2 intervals; however, there was a bias of responses towards the unilateral side by test interval III. Conclusions: The lateralization abilities of children with bilateral CIs develop in a relatively short period of time (1-2 years) after the second implant. Children appear to be able to acquire binaural skills after bilateral cochlear implantation.
Surgical management of lateral neck abscesses in children: a retrospective analysis of 100 cases

Cervical abscesses are relatively common infections in pediatric patients. There is an ongoing debate about the necessity and time point of surgical drainage. The identification of a focus of infection might play an important role in facilitating a therapeutic decision. In a retrospective study, 100 pediatric patients aged 1–18 years who underwent incision and drainage of a lateral cervical abscess at our institution were analyzed. Patients were divided into two groups based on whether a focus of infection could be identified or not. Data collection included patient characteristics, microbiological results, antibiotic regimen, and clinical course. A focus of infection was found in 29% (29/100) of the patients, most frequently in the tonsils. A causative microorganism was found in 75% (75/100) of all patients, with Staphylococcus aureus and Streptococcus pyogenes being the most common pathogens. All patients received an empiric antibiotic therapy in addition to surgery. Antibiotic medication was changed in 31% in both groups (9/29 with a focus of infection and 22/71 without a focus of infection) during therapy. Children without an identified focus of infection generally were younger and had more comorbidities reducing immune response while also showing differences in the pathogens involved. There were no complications associated to surgery or antibiotic therapy in any of the patients involved.
Conclusion: Children with an identified focus of infection show several differences compared to those with isolated lateral abscesses, especially regarding the microorganisms involved. But the focus of infection seems not to have an impact on patient’s outcome.

Zu den erstaunlichsten Eigenschaften des auditorischen Systems zählt die Fähigkeit Sprachsignale auch in lauter Umgebung wahrnehmen, verarbeiten und verstehen zu können. Diese Fähigkeit bildet zusammen mit der Sprachproduktion die Grundlage der menschlichen audio-verbalen Kommunikation. Schließlich spielen sich Gespräche im Alltag selten in absoluter Ruhe ab. Hörgesunde Menschen sind sogar in der Lage in Anwesenheit intensiver Störgeräusche wie Verkehrslärm oder Stimmengewirr eine Unterhaltung zu führen Die vorliegende Arbeit sollte klären, inwieweit der Würzburger Kindersprachtest für die Anwendung im Störgeräusch geeignet ist. Ein weiteres Ziel war das Sprachverstehen im Störlärm von Kindern im Alter von fünf bis neun Jahren mit Hilfe des Würzburger Kindersprachtests zu untersuchen. Insgesamt nahmen 24 Kinder im Alter von 5 bis 9 Jahren an der Studie teil. Davon waren 11 Mädchen und 13 Jungen. Das Durchschnittsalter betrug 6,8 Jahre. Als Testmaterial diente der Test IV des Würzburger Kindersprachtests (WueKi), bestehend aus 3 Listen zu je 20 zweisilbigen Wörtern und als Störgeräusch das Würzburger Rauschen, das durch zeitversetzte 32fache Überlagerung des gesamten Sprachmaterials generiert wurde. Der Test ließ sich in allen Fällen problemlos bei guter Akzeptanz seitens der Kinder durchführen. Für jedes Kind gelang es, bei einem Signalpegel von 60 dB die individuelle Sprachverständlichkeitsschwelle (speech reception threshold, kurz SRT) in Abhängigkeit des Signal-Rauschabstands (signal-to-noise ratio, kurz SNR) zu bestimmen. Sie betrug im Durchschnitt -8,3 dB SNR, -7,7 dB SNR für die Liste 1, -9,2 dB SNR für die Liste 2 und -8,1 dB SNR für die Liste 3. Es ließ sich ein geringer, nicht signifikanter Alterseffekt zu Gunsten der älteren Kinder nachweisen. Die Ergebnisse wurden in zwei Gruppen eingeteilt. In Gruppe 1 fiel der Schwierigkeitsgrad der Höraufgabe ab, während er in Gruppe 2 anstieg. Die SRT betrug in Gruppe 1 -6,4 dB SNR und in Gruppe 2 -9,2 dB SNR. Insgesamt ist der Würzburger Kindesprachtest geeignet, das individuelle Sprachverständnis eines Kindes im Störlärm zu bestimmen und stellt deshalb ein sinnvolles Instrument bei der Anpassung und Verlaufsbeurteilung moderner Hörhilfen dar. Ebenso ist ein Einsatz in der Diagnostik von Hörstörungen denkbar.
