Studies on receptor signaling and regulation in platelets and T cells from genetically modified mice

Receptors with tyrosine-based signaling motifs control essential functions of hematopoietic cells, including lymphocytes and platelets. Downstream of the platelet receptor glycoprotein (GP) VI and the T cell receptor (TCR) the immunoreceptor tyrosine-based activation motif (ITAM) initiates a signaling cascade that involves kinases, adapter and effector proteins and finally leads to cellular activation. This thesis summarizes the results of three studies investigating different aspects of receptor signaling and regulation in platelets and T cells.
In the first part, the impact of constitutive Ca2+ influx on TCR signaling and T cell physiology was investigated using a transgenic mouse line with a mutation in the Ca2+ sensor stromal interaction molecule 1 (STIM1). The elevated cytoplasmic Ca2+ level resulted in an altered phosphorylation pattern of the key enzyme phospholipase (PL) Cγ1 in response to TCR stimulation, but without affecting its enzymatic activity. Withdrawal of extracellular Ca2+ or inhibition of the phosphatase calcineurin restored the normal phosphorylation pattern. In addition, there was a decrease in the release of Th2-type cytokines interleukin 4, 5 and 13 upon stimulation in vitro.
The second part of the thesis deals with the role of the adapter protein growth factor receptor-bound protein 2 (Grb2) in platelets using a megakaryocyte/platelet-specific knockout mouse line. Loss of Grb2 severely impaired signaling of GPVI and C-type lectin-like receptor 2 (CLEC-2), a related hemITAM receptor. This was attributed to defective stabilization of the linker for activation of T cells (LAT) signalosome and resulted in reduced adhesion, aggregation, Ca2+ mobilization and procoagulant activity downstream of (hem)ITAM-coupled receptors in vitro. In contrast, the signaling pathways of G protein-coupled receptors (GPCRs) and the integrin αIIbβ3, which do not utilize the LAT signalosome, were unaffected. In vivo, the defective (hem)ITAM signaling caused prolonged bleeding times, however, thrombus formation was only affected under conditions where GPCR signaling was impaired (upon acetylsalicylic acid treatment). These results establish Grb2 as an important adapter protein in the propagation of GPVI- and CLEC-2-induced signals.
Finally, the proteolytic regulation of the immunoreceptor tyrosine-based switch motif (ITSM)-bearing receptor CD84 in platelets was investigated. This study demonstrated that in mice CD84 is cleaved by two distinct and independent proteolytic mechanisms upon platelet activation: shedding of the extracellular part, which is exclusively mediated by a disintegrin and metalloproteinase (ADAM) 10 and cleavage of the intracellular C-terminus by the protease calpain. Finally, the analysis of soluble CD84 levels in the plasma of transgenic mice revealed that shedding of CD84 by ADAM10 occurs constitutively in vivo.
In der vorliegenden Arbeit werden Studien zu verschiedenen Aspekten der Stimulation von humanen Vg9Vd2-T-Lymphozyten vorgestellt. Ein Schwerpunkt war die Charakterisierung der Erkennung von Bisphosphonaten durch Vg9Vd2-T-Zellen. Weder die Alkylmonophosphonate Ethyl- und Propylphosphonat noch 3-Aminopropylphosphonat bewirkten eine Stimulation von Vg9Vd2 T-Lymphozyten. Anscheinend ist also für die gd-T-Zell-stimulierende Aktivität von Aminobisphosphonaten das Vorhandensein sowohl der Methylenbisphosphonat-Gruppe als auch des Amino-Stickstoffs entscheidend. Es wurden verschiedene Pamidronat-Derivate untersucht, die sich durch Substituenten am Stickstoffatom unterscheiden. Die meisten dieser Verbindungen konnten Vg9Vd2-T-Zellen aktivieren, die Art der Substituenten hatte aber großen Einfluss darauf, welche Konzentration des jeweiligen Bisphosphonats für eine gd-T-Zell-Antwort nötig war. Besonders negativ auf die gd-T-Zell-stimulierende Aktivität wirkte sich aus, wenn das Stickstoffatom Teil einer Säureamidbindung war. Das lässt darauf schließen, dass die Gegenwart einer positiven Ladung (durch Protonierung des Stickstoffatoms) von Bedeutung für die Bioaktivität dieser Verbindungen ist. Beim Vergleich verschiedener Bisphosphonate mit stickstoffenthaltenden Heteroaromaten (Fünfringe mit ein bis drei Stickstoffatomen) zeigte sich, dass sowohl die Position des basischen Stickstoffatoms im Ring als auch Art und Position von Ringsubstituenten Einfluss auf deren gd-T-Zell-stimulierende Aktivität haben. Es ergaben sich Hinweise, dass sich eine gesteigerte Neigung eine positive Ladung in der Seitenkette zu tragen bei diesen Verbindungen genau wie bei Aminobisphosphonaten günstig auf das gd-T-Zell-aktivierende Potential auswirkt. Durch Behandlung mit Zoledronat wurde die monozytäre Zelllinie THP-1 stimulierend für Vg9Vd2-T-Zellen. Auch weitere Zelllinien und Lymphozyten des peripheren Bluts (PBL) konnten nach Vorinkubation mit Zoledronat Vg9Vd2-T-Zellen aktivieren. Dabei genügten bei den PBL deutlich geringere Zoledronat-Konzentrationen um einen Effekt zu erzielen als bei den untersuchten Zelllinien. Für die indirekte Stimulation von Vg9Vd2-T-Zellen durch Zoledronat mittels THP-1 Zellen oder PBL war Zell-Zell-Kontakt zwischen den präsentierenden Zellen und den gd-T-Zellen Voraussetzung. Die Anwesenheit von alkalischer Phosphatase hatte keine Auswirkungen auf die gd-T-Zell-Aktivierung durch Zoledronat. Dies spricht dafür, dass Vg9Vd2-T-Zellen Oberflächenstrukturen auf anderen Zellen erkennen und dass freie Phosphoantigene bei der gd-T-Zell-Stimulierung durch Stickstoff enthaltende Bisphosphonate keine Rolle spielen. Wurde die Vorinkubation von THP-1 Zellen mit Zoledronat in Gegenwart von Saponin, einem Detergenz das die Durchlässigkeit der Zellmembran reversibel erhöht, durchgeführt, reichten deutlich niedrigere Konzentrationen des Bisphosphonats aus, um die THP-1 Zellen stimulierend für gd-T-Lymphozyten zu machen. Das ist ein Hinweis darauf, dass für die Aktivierung von Vg9Vd2-T-Zellen durch Zoledronat intrazelluläre Vorgänge in den „antigenpräsentierenden“ Zellen verantwortlich sein könnten. Beim Vergleich verschiedener N-BPs hinsichtlich ihrer Aktivität gegenüber gd-T-Zellen und ihrer antiresorptiven Potenz ergab sich eine erstaunlich gute Korrelation. Dies könnte darauf hinweisen, dass beide Effekte durch den gleichen Mechanismus – die Hemmung der Farnesylpyrophosphat-Synthase – zustande kommen. Die Gegenwart von Farnesol oder Geranylgeraniol während der Vorinkubation von THP-1 Zellen mit Zoledronat verringerte deren gd-T-Zell-stimulierendes Potential nicht, so dass vielleicht nicht die Verarmung an längerkettigen Isoprenoiden sondern die Anreicherung von Vorläufern zu Veränderungen in den Zellen führt, die diese schließlich erkennbar für Vg9Vd2-T-Zellen machen. Zusätzlich wurden im Rahmen dieser Arbeit einige Versuche zu bakteriellen Phosphoantigenen durchgeführt. Mittels einer selektiven HPLC-MS/MS-Methode gelang uns in einer E. coli-Probe der Nachweis eines Vg9Vd2-T-Zell-stimulierenden Pyrophosphats mit der molekularen Masse von 3-Formyl-1-butyl-pyrophosphat, einem Phosphoantigen das in Mykobakterien gefunden worden war (Belmant und Mitarbeiter, 1999). Eine weitere Strukturaufklärung war aufgrund der äußerst geringen Konzentration des Phosphoantigens nicht möglich. 2-C-Methyl-D-erythritol-2,4-cyclodiphosphat (MEcPP) ist ein Zwischenprodukt des 2-C-Methylerythritol-4-phosphat- (MEP) Wegs der Isoprenoidbiosynthese. Es wird in manchen Bakterien – z. B. Corynebacterium ammoniagenes – bei oxidativem Stress verursacht durch Benzylviologen (BV) akkumuliert. Es konnte gezeigt werden, dass Extrakte aus C. ammoniagenes, die in Gegenwart von BV kultiviert wurden, in einem höherem Maße Vg9Vd2-T-Zellen stimulieren als Proben von Bakterien, die ohne BV-Zusatz wuchsen; MEcPP wirkte selbst nicht als Phosphoantigen. Dies war ein weiterer Hinweis darauf, dass bakterielle Phosphoantigene mit dem MEP-Weg assoziiert sind.
Transforming-Growth-Factor-beta1 (TGF-b1) is a multifunctional cytokine that regulates cell growth and differentiation in many types of cells. TGF-b1 is especially known to exert a variety of regulatory functions in the immune system, such as T cell differentiation and T cell function. Signal transduction of TGF-b1 is mediated by phosphorylation of receptorassociated Smad proteins (R-Smads). R-Smads are phosphorylated by the activated type I receptor, which is itself phosphorylated by the high affinity type II receptor upon ligand binding. The phosphorylated R-Smads then associate with Co-Smads. Heterooligomers of R- and Co-Smads translocate into the nucleus where they regulate transcription of target genes in concert with other transcription factors such as CBP/p300 or AP-1. Recent findings suggest that the pleiotropic effects of TGF-b1 are conferred by crosstalks to other signal transduction pathways such as the MAP-kinases or the STAT-pathway. Here we describe the effect of long-term exposure to TGF-b1 on the effector function of differentially stimulated primary murine splenocytes and purified primary murine CD8+ cytotoxic T cells. Long-term exposure to TGF-b1 results in non-responsiveness to TGF-b1- induced Smad2 phosphorylation. This is seen either by no phosphorylation or sustained phosphorylation of Smad2. Furthermore, we observed a strong correlation between sustained Smad2 phosphorylation and resistance to TGF-b1 mediated growth inhibition. In contrast, splenocyte cultures strongly growth inhibited by TGF-b1 showed no Smad2 phosphorylation. Lytic activity of these cultures, however, was found to be suppressed regardless of proliferation properties and Smad2 phosphorylation pattern. We also describe that a functional MEK-1 pathway is a prerequisite for rendering murine splenocytes unresponsive to TGF-b1 mediated growth inhibition, and that inhibition of the MEK-1 cascade alters the Smad2 phosphorylation pattern. In addition, we show that resistance to TGF-b1 mediated growth inhibition correlates with the activation of the JNK pathway. However, the resistant phenotype was found unable to be reverted upon administration of exogeneous IFNg and/or aCD28 antibody. In human or mouse T cell lines, however, the described correlation between the type of stimulation and TGF-b growth resistance or growth sensitivity is not present. Thus, this correlation is specific for primary T cells. We also cloned a chimeric dominantnegative TGF-b receptor which is coupled to a suicide gene, in order to render T cells resistant to TGF-b mediated effects.These findings shed light on how TGF-b1 mediates its immunosuppressive role, and may help to gain knowledge of averting these TGF-b1 effects in the course of tumor therapy.
Rolle der gammadelta T-Zellen in der Immunantwort bei Patienten mit gastrointestinalen Tumoren

Zusammenfassend konnte im Rahmen der vorliegenden Arbeit die Frage nach der Fähigkeit der selektiven Stimulierung mittels des Phosphorantigens HMBPP und den beiden BTN3 Antikörpern bestätigt werden. Es konnte zudem wie erwartet hierbei ein Unterschied zwischen den beiden Kohorten detektiert werden. Dabei zeigte die Kohorte der Normalspender erwartungsgemäß eine stärkere Aktivierungs- sowie Proliferations-fähigkeit. Normalspender ließen sich signifikant besser mit HMBPP aktivieren und bei bestimmter Konzentration signifikant besser proliferieren, bei BTN3A und sc20.1 konnten keine signifikanten Unterschiede ermittelt werden, allerdings anhand der Mittelwerte eine deutlich stärkere Aktivierung und Proliferation aufgezeigt werden. Außerdem konnten interessante interindividuelle Unterschiede detektiert werden, die neue Erkenntnisse brachten. Mit Hilfe der untersuchten Oberflächenmoleküle CD45RA und CD27 und der Einteilung der gammadelta T-Zellen in unterschiedliche Subgruppen konnten so mögliche Erklärungen für die Unterschiede zwischen den Kohorten aufgezeigt werden. Normalspender zeigten signifikant höhere Anteile an naiven gammadelta T-Zellen und nicht signifikant höhere Anteile an central memory T-Zellen, demnach eine deutliche Verschiebung in Richtung nicht differenzierter Subsets, wohingegen die Tumorkohorte signifikant höhere effector memory T-Zellen aufwiesen und somit eine deutliche Verschiebung in Richtung differenzierter Subsets. Dadurch kann erklärt werden, weshalb Normalspender besser aktiviert werden und besser proliferieren können. Auch die Einteilung in unterschiedliche Profile 1-6 anhand CD28, CD27 und CD16 lieferte Gründe für den Unterschied zwischen den Kohorten, wobei Normalspender der Gruppe 1 und 2, Tumorpatienten der Gruppe 3 und 4 angehörten. Durch Ermittlung weiterer signifikanter Änderungen einiger exprimierter Oberflächenmoleküle CD39, CD161 und PD1 wurde mit Hilfe der vorliegenden Arbeit bekräftigt, dass einige Faktoren betrachtet werden müssen, die die Proliferation und Aktivierung der gammadelta T-Zellen positiv und negativ beeinflussen können. Es konnte jedoch auch erneut verdeutlicht werden, wie komplex und weitgreifend der Aktivierungsmechanismus, die damit verbundene Expansion und die Auslösung der einzelnen Effektorfunktionen ist.
Leishmaniasis is a neglected tropical disease that can be manifested through different clinical forms, ranging from cutaneous to visceral. The host response against Leishmania spp. is greatly dependent on T cell-mediated immunity, in which T helper 1 responses are associated with macrophage activation and elimination of the parasite, while regulatory T cells and T helper 2 responses are correlated with parasite survival and persistence of infection. Leishmania uses different virulence factors as strategies for evading the immune response of the host. One of them are cathepsin-like cysteine proteases, which are currently under extensive investigation as targets for drug development. Previous studies with inhibitors of cathepsins B and L in vivo revealed an outstanding modulation of the host T helper cell response. However, the mechanisms behind these observations were not further investigated. Given the urgent need for better treatments against leishmaniasis, the aim of this study was to investigate the effects that the lack of cathepsin B and L activity have on the signals that dendritic cells use to instruct T helper cell polarization in response to infection with Leishmania major.
The cathepsin inhibitors tested showed low or no cytotoxicity in bone marrow-derived dendritic cells, and dendritic cells and macrophages could be generated from cathepsin B and cathepsin L-deficient mice without apparent alterations in their phenotype in comparison to wild-type controls. Furthermore, lack of cathepsin B and L activity showed no impact in the rate of promastigote processing by dendritic cells. Cathepsin B and cathepsin L-deficient macrophages showed no differences in parasite proliferation and capacity to produce nitric oxide in comparison to wild-type macrophages. In response to the parasite, dendritic cells treated with a cathepsin B inhibitor and dendritic cells from cathepsin B-deficient mice showed higher levels of expression of major histocompatibility complex (MHC) class II molecules than dimethyl sulfoxide (DMSO) or wild-type controls, but it was not accompanied by changes in the expression of costimulatory molecules. Wild-type dendritic cells and macrophages are not able to express the pro-inflammatory cytokine interleukin (IL)-12 in response to promastigotes. However, cells treated with a cathepsin B inhibitor or cells deficient for cathepsin B were able to express IL-12, whilethe expression of other cytokines -including IL-6 and tumor necrosis factor (TNF)-alpha-remained unchanged. These characteristics point towards a more “pro-Th1” profile of dendritic cells in the absence of cathepsin B.
This data is the first report on IL-12 regulation depending on cathepsin B. The IL-12 up-regulation observed was already present at the transcriptional level. Furthermore, it was also present in macrophages and dendritic cells in response to LPS, and the latter had a higher capacity to induce T cell helper 1 polarization in vitro than wild-type dendritic cells. The activation of different signaling pathways was analyzed, but the up-regulation of IL-12 could not be attributed to modulation of nuclear factor-kappaB (NFkappaB), p38 mitogen activated protein kinase (MAPK) and extra-cellular signal-regulated kinase (ERK)1/2 pathways. Thus, the mechanism behind IL-12 regulation by cathepsin B remains to be elucidated, and the impact of these effects is yet to be confirmed in vivo. Altogether it is tempting to speculate that cathepsin B, in addition to its role in processing endocytosed material, is involved in the modulation of the pro-inflammatory cytokine IL-12.
Cardiac healing after myocardial infarction (MI) represents the cardinal prerequisite for proper replacement of the irreversibly injured myocardium. In contrast to innate immunity, the functional role of adaptive immunity in postinfarction healing has not been systematically addressed. The present study focused on the influence of CD4+ T lymphocytes on wound healing and cardiac remodeling after experimental myocardial infarction in mice. Both conventional and Foxp3+ regulatory CD4+ T cells (Treg cells) became activated in heart draining lymph nodes after MI and accumulated in the infarcted myocardium. T cell activation was strictly antigen-dependant as T cell receptor-transgenic OT-II mice in which CD4+ T cells exhibit a highly limited T cell
receptor repertoire did not expand in heart-draining lymph nodes post-MI. Both OT-II and major histocompatibility complex class II-deficient mice lacking a CD4+ T cell compartment showed a fatal clinical postinfarction outcome characterized by disturbed scar tissue construction that resulted in impaired survival due to a prevalence of left-ventricular ruptures. To assess the contribution of anti-inflammatory Treg cells on wound healing after MI, the Treg cell compartment was depleted using DEREG mice that specifically express the human diphtheria toxin receptor in Foxp3-positive cells, resulting in Treg cell ablation after diphtheria toxin administration. In a parallel line of experiments, a second model of anti-CD25 antibody-mediated Treg cell immuno-depletion was used. Treg cell ablation prior to MI resulted in adverse postinfarction left-ventricular dilatation associated with cardiac deterioration. Mechanistically, Treg cell depletion resulted in an increased recruitment of pro-inflammatory neutrophils and Ly-6Chigh monocytes into the healing myocardium. Furthermore, Treg cell-ablated mice exhibited an adverse activation of conventional non-regulatory CD4+ and CD8+ T cells that
showed a reinforced infiltration into the infarct zone. Increased synthesis of TNFα and IFNγ by conventional CD4+ and CD8+ T cells in hearts of Treg cell-depleted mice provoked an M1-like macrophage polarization characterized by heightened expression of healing-compromising induced NO synthase, in line with a reduced synthesis of healing-promoting transglutaminase factor XIII (FXIII), osteopontin (OPN) and transforming growth factor beta 1 (TGFβ1).
Therapeutic Treg cell activation by a superagonistic anti-CD28 monoclonal antibody stimulated Treg cell accumulation in the infarct zone and led to an increased expression of mediators inducing an M2-like macrophage polarization state, i.e. interleukin-10, interleukin-13 and TGFβ1. M2-like macrophage differentiation in the healing infarct was associated with heightened expression of scar-forming procollagens as well as scar-stabilizing FXIII and OPN, resulting in improved survival due to a reduced incidence of left-ventricular ruptures. Therapeutic Treg cell activation and the induction of a beneficial M2-like macrophage polarization was further achieved by employing a treatment modality of high clinical potential, i.e. by therapeutic administration of IL-2/ anti-IL-2 monoclonal antibody complexes. The findings of the present study suggest that therapeutic Treg cell activation and the resulting improvement of healing may represent a suitable strategy to attenuate adverse infarct expansion, left-ventricular remodeling, or infarct ruptures in patients with MI.
Peripheral blood mononuclear cells (PBMCs) are the only source of human lymphoid cells routinely available for immunologic research and for immunomonitoring of T-cell responses to microbial and tumor-associated antigens. However the large majority of human T-cells resides in tissues, especially in lymphatic organs, while only 1 % of the body’s T-cells circulate in the blood stream. Previous work in mice and humans had indicated that CD4 T-cells transiently lose antigen sensitivity when cellular contacts are lost, e.g. by leaving lymphoid organs such as lymph nodes (LNs) and entering the circulation. In this study, these findings were extended to CD8 T-cells. Thus, CD8 T-cell responses of the human tonsil show a significant drop in sensitivity to viral antigens if tissue-exit was simulated by keeping cells in dispersed culture at body temperature for two hours.
Conversely, tissue-like functionality in blood-derived CD8 T-cells was restored by applying the simple and robust RESTORE protocol. Indeed, application of the RESTORE protocol, i.e. pre-culturing PBMCs for two days at a high cell density before initiation of antigenic stimulation, demonstrated that CD8 T-cell responses to a broad range of viral and to tumor-associated antigens are greatly underestimated, and sometimes even remain undetected if conventional, unprocessed PBMC cultures are used. The latter finding is particularly striking with regard to the appearance of Wilms tumor 1 (WT1)-specific CD8 T-cell responses in leukemia patients after allogeneic bone marrow transplantation. My studies on the mechanism of the RESTORE protocol show that HD preculture of PBMCs does not involve antigen-or cytokine-driven clonal expansion of T-cells. Moreover, the gain in antigen sensitivity cannot be explained by a decreased activity of regulatory T-cells during the preculture step. The increased antigen sensitivity of CD8 T-cells from HD precultures of PBMCs is associated with tonic T-cell receptor signaling as indicated by enhanced tyrosine phosphorylation of the CD3 ζ chains and the tyrosine kinase Lck, thereby preparing T-cells for full responses. The upregulation of genes involved in aerobic glycolysis in “restored” CD8 memory T-cells relative to fresh cells might be an essential requirement for increased T-cell functionality including the regulation of IFN-γ production. Taken together, the RESTORE protocol, which was initially described for the CD4 T-cell response to the antibody TGN1412 permits a more meaningful monitoring of CD8 T-cell responses to viral infections and tumors. Furthermore, when generating T-cell lines for adoptive T-cell therapy, the RESTORE protocol allows the generation of CD8 T-cell lines with an improved representation of clones responding to low antigen concentrations.
Die Unterscheidung zwischen körpereigenen und körperfremden Strukturen ist eine grundlegende Herausforderung der spezifischen Immunantwort. Pathologische Veränderungen dieser Abgrenzung können zu schwerwiegenden Autoimmunerkrankungen wie beispielsweise Diabetes Mellitus, Rheumatischer Arthritis oder Multipler Sklerose führen. Um unerwünschte (Auto-) Immunreaktionen zu verhindern, existieren verschiedene Formen von peripheren Toleranzmechanismen, die durch viele Transkriptionsfaktoren wie z. B. ICER (inducible cAMP early repressor), NFAT (nuclear factor of activated T cells) und Foxp3 (forkhead box protein p3) kontrolliert werden. Foxp3+ regulatorische T-Zellen (Tregs) sind spezialisierte immun-suppressive Lymphozyten, welche die Aktivierung anderer Immunzellen unterdrücken können. Einer der möglichen Mechanismen ist der Transfer zyklischen Adenosin-Monophosphats (cAMP) von Tregs in konventionelle T- und B-Lymphozyten. Die erhöhte intrazelluläre Konzentration an cAMP führt in Effektorzellen zur Induktion und Kerntranslokation von ICER. Der transkriptionelle Repressor ICER supprimiert die Expression vieler NFAT-regulierter Gene und hemmt darüber hinaus die Induktion der NFATc1/αA-Isoform selbst. Diese Isoform wird speziell in pro-inflammatorischen Effektorzellen hochreguliert und ist maßgeblich an deren spezifischem transkriptionellen Programm beteiligt. Foxp3 ist ein zentraler Faktor für die Bildung und Funktion sowohl Thymus-generierter nTregs als auch peripher (TGFβ-) induzierter iTregs. Die Kontrolle des Foxp3-Gens wird in iTregs – überraschenderweise aber nicht in nTregs – durch NFAT-Faktoren reguliert. Allerdings hemmt Foxp3 durch eine negative Rückkopplung wiederum die Induktion und Aktivität von NFATc1/αA. Dies stellt somit ein weiteres Regulativ dar, wobei Foxp3 nicht nur die Plastizität, sondern auch die Funktion von immun-suppressiven T-Zellen steuert. Zusätzlich regulieren die verschiedenen NFAT-Faktoren auch die Antigen präsentierenden dendritischen Zellen (DCs). Während NFATc1 und NFATc2 die Differenzierung und Proliferation von DCs beeinflussen, reguliert NFATc3 deren Zytokinexpression und steuert indirekt auch die nachfolgende T-Zell-Immunantwort. Die Kontrolle der Genregulation in Immunzellen durch die Transkriptionsfaktoren ICER, NFAT und Foxp3 erfüllt somit spezifische Funktionen der Immunität, reguliert aber gleichzeitig wichtige Aspekte der peripheren Toleranz, um schädliche (Auto-) Immunreaktionen zu verhindern.
Diabetes mellitus Typ 1 ist eine chronische Autoimmunkrankheit, bei der die Beta-Zellen der Langerhans-Inseln im Pankreas durch autoreaktive T Lymphozyten zerstört werden und somit die Insulinproduktion zum Erliegen kommt.
Die vorliegende prospektive Querschnittsstudie untersucht die die Reaktion und Polarisierbarkeit der peripheren T Lymphozyten, die Zytokinproduktion der PBMCs und die Expression der spezifischen Transkriptionsfaktoren Tbet (Th1), FoxP3 (Th17) und RORc (Treg) nach Stimulation mit diabetesspezifischen Antigenen und Candida albicans bei sieben gesunden Kontrollen, neun erstmanifestierten T1DM Patienten und elf langzeiterkrankten T1DM Patienten.
Bei der Untersuchung der spezifischen CD4+ T Zellen zeigte sich, dass EM sowohl im unstimulierten Ansatz, aber auch nach Stimulation mit den Antigenen eine größere Proliferationsaktivität aufwies als HD und LS. Interessanterweise war ein vergleichbarer Unterschied bei den CD8+ T Zellen lediglich nach Stimulation mit GAD65 zu sehen. Bei Betrachtung der CD4+ Subpopulationen erkennt man, dass es in allen Kohorten große interindividuelle Unterschiede gibt und man keine signifikanten Unterschiede zwischen den Kohorten beobachten kann. Trotzdem lässt sich sagen, dass sich die Subpopulationen nach den spezifischen Stimulationen in den drei Kohorten teilweise unterschiedlich verschieben und dies Anzeichen dafür ist, dass die T Zellen der T1DM Patienten auf diese Antigene anders verhalten als HD. Bei den CD8+ TEMRAs gibt es mehrere signifikante Unterschiede und es fällt auf, dass EM deutlich weniger TEMRA aufweisen als die anderen beiden Kohorten. Sowohl bei den CD25+ Tregs als auch bei den CD161+ Th17 Zellen zeigen sich keine relevanten Signifikanzen.
Die Chemokinrezeptor tragenden weisen sowohl bei den CD4+-T Zellen als auch bei den CD8+ T Zellen Unterschiede und auch Parallelen zwischen den Kohorten auf. Während sich die CD4+CCR5+ Th1 Zellen bei EM durch die Antigene polarisieren lassen, findet bei HD keine Polarisierung statt. Dafür tragen die HD über allen Ansätzen mehr CD4+ und CD8+CXCR3+ Th1 Zellen als EM. Bei den Chemokinrezeptoren CCR6+ und CD25+CCR5+ zeigen sich keine bemerkenswerten Unterschiede oder Polarisierungen durch die Antigene.
Im Einklang mit den Ergebnissen der Phänotypisierung der Th1, Th17 und Treg Zellen stehen die der spezifischen Transkriptionsfaktoren. Auch hier waren keine signifikanten Unterschiede zwischen den drei Kohorten vorhanden. Interessanterweise zeigte sich jedoch, dass die relative Transkription nach Stimulation mit den Antigenen in allen drei Kohorten fast ausnahmslos abnahm. Abschließend ist zu erwähnen, dass die Probandenzahl bei dieser Untersuchung sehr klein war und große interindividuelle Unterschiede vorlagen.
Bei Betrachtung des Th1 spezifischen Zytokins IFN y fiel auf, dass HD und LS im unstimulierten Ansatz deutlich mehr produzierte als EM, während die Konzentrationen durch Stimulation mit den diabetesspezifischen Antigenen bei HD und LS stark abfiel und bei EM annähernd gleichgeblieben ist. Auffallend war außerdem, dass die durchschnittliche Produktion des Th17 spezifischen Zytokins IL 17 von EM in vielen Ansätzen deutlich größer war als von HD. Das hauptsächlich von den Treg Zellen produzierte IL-10 war bei den T1DM Patienten deutlich kleiner als bei HD. Ebenso wie IFN-y fiel die Konzentration durch die Stimulationen bei HD jedoch stark ab, während sie bei EM und LS gleichblieb oder anstieg. Insgesamt lässt sich sagen, dass es große interindividuelle Unterschiede innerhalb der Kohorten hinsichtlich der Produktion der Zytokine in den verschiedenen Ansätzen gab. Somit ist es von enormem Interesse, die Zytokinproduktion nach Stimulation mit den diabetesspezifischen Antigenen in den verschiedenen Kohorten an einer größeren Anzahl an Probanden zu untersuchen.
Zusammenfassend ergeben sich einzelne Hinweise, dass sich die Reaktion der T Zellen auf diabetesspezifische Antigene bei erstmanifestierten T1DM von HD unterscheiden. Inwieweit einzelne Autoantigen-spezifische Subpopulationen, Transkriptionsfaktoren oder proinflammatorische bzw. antiinflammatorische Zytokine eine Rolle in der Pathogenese des T1DM spielen und diese ein Angriffsziel für Therapeutika sein könnten, gilt es weiterhin in humanen Studien herauszufinden.
Die chronische Rhinosinusitis (CRS) ist eine entzündliche Erkrankung der Nase und Nasennebenhöhlen mit einem Bestehen von Symptomen über zwölf Wochen im Jahr. In Europa sind über 10% der erwachsenen Bevölkerung von ihr betroffen und leiden unter einer stark verminderten Lebensqualität. Nach dem Phänotyp lassen sich zwei Formen unterscheiden: die chronische Rhinosinusitis mit Polypen (CRScNP) und ohne Polypen (CRSsNP). Meist wird unter europäischen Probanden bei der CRScNP eine hauptsächlich eosinophile Th2- vermittelte und bei der CRSsNP ein gemischtes Profil aus Th1/Th17/Th2-vermittelter Entzündung beschrieben. Doch dies erfasst nicht die Vielfalt dieser Erkrankung. Erst kürzlich konnten Endotypen, basierend auf der Zytokinsekretion, identifiziert werden, die auf ein multifaktorielles Geschehen schließen lassen. Nachdem im Nasensekret von über 90% der CRS Patienten Pilze nachgewiesen werden konnten erscheint es wichtig, deren Rolle in der Pathogenese der CRS zu ergründen. Besonders interessant sind dabei Aspergillus fumigatus und Candida albicans. Als opportunistische Pilze könnten sie im Rahmen der CRS in der Lage sein bei einigen Endotypen durch eine verstärkte Aktivierung von Immunzellen die lokale Entzündungsreaktion aufrecht zu erhalten. Es wurden lokale und periphere CD4+ und CD8+ T-Zellen von Patienten mit CRScNP, CRSsNP und einer gesunden Kontrollgruppe isoliert. Eine Kultivierung mit den Antigenen von C. albicans und A. fumigatus erfolgte in einer Konzentration von 1 µg/ml über 6 Tage. Im Anschluss daran wurden die Zellen für das Durchflusszytometer angefärbt und ihre Aktivität über die Ki67+ Expression gemessen. In den Überständen der Kultivierung wurde durch das Durchflusszytometer mittels Multiplexassay die produzierten Zytokinkonzentrationen der Zellen bestimmt.
