Universal prevention for non-suicidal self-injury in adolescents is scarce - A systematic review

Non-suicidal self-injury (NSSI) during adolescence is a high-risk marker for the development and persistence of mental health problems and has been recognized as a significant public health problem. Whereas targeted prevention has indeed shown to be effective in reducing NSSI and improve mental health problems, access to such programs is limited. By face validity, universal prevention of NSSI seems an ideal starting point for a stepped-care model to circumvent a lack of resources in the medical care system. However, it is yet unclear how effective such approaches are. Here, we provide a summary of existing work on universal prevention of NSSI in adolescents younger than 21 years based on a systematic literature search. We found that only seven studies are available. None of the programs evaluated was found to be effective in reducing the incidence or frequency of NSSI. After providing a comprehensive summary of the existing work, we evaluate the fact that existing work primarily focusses on selected/targeted prevention and on psychoeducational methods. We derive implications for future directions in the field of universal prevention of NSSI.
In der vorliegenden Arbeit sollte im Rahmen einer Metaanalyse die Effektivität der verschiedenen präventiven Stabilisierungshilfen auf das Sprunggelenk bewertet werden. Dazu wurde in den medizinischen Datenbanken Medline und Pubmed nach relevanten Studien recherchiert. Nach der Literaturselektion entsprechend festgelegter Auswahlkriterien konnten 44 Studien im Zeitraum von 1962 bis 2005 in die Bewertung einfließen. Diese wurden der Evidenzhierarchie nach der Cochrane Collaboration zugeordnet. Entsprechend der Evidenzstärken und der kritischen Beurteilung der externen und internen Validität wurden die einzelnen Stabilisierungshilfen bewertet. Dabei zeigt sich, dass ältere, weit verbreitete und langzeiterprobte Maßnahmen wie der adhäsive Tape- Verband innovativeren und ausbaufähigen Methoden wie dem propriozeptiven Training weichen. In diesem sensomotorischen Bereich konnten übereinstimmend positive und größtenteils signifikante Ergebnisse ermittelt werden. Auch die Anwendung semirigider und rigider Orthesen zeigte bei der Mehrzahl der Studien einen signifikanten Supinationsschutz. Der präventive Effekt von (Schnür-) Bandagen äußerte sich vornehmlich in der Verbesserung der propriozeptiven Fähigkeiten vor allem instabiler Sprunggelenke. Beim Tape-Verband steht die initiale signifikante Supinationsrestriktion im Vordergrund, was unter anderem mit den Materialeigenschaften sowie vielfältigen und eingeschränkt reproduzierbaren Techniken begründet wird. Die Untersuchungen zu Schuhen unterschiedlicher Schafthöhen konnten keine übereinstimmend signifikanten Ergebnisse liefern.
Background: Recent studies demonstrated that engagement of sodium glucose transporter 1 (SGLT-1) by orally administered D-glucose protects the intestinal mucosa from lipopolysaccharide (LPS)-induced injury. We tested whether SGLT-1 engagement might protect the intestinal mucosa from doxorubicin (DXR)- and 5-fluorouracil (5-FU)-induced injury in animal models mimicking acute or chronic mucositis.
Methods: Mice were treated intraperitoneally with DXR, alone or in combination with 5-FU, and orally with BLF501, a glucose-derived synthetic compound with high affinity for SGLT-1. Intestinal mucosal epithelium integrity was assessed by histological analysis, cellular proliferation assays, real-time PCR gene expression assays and Western blot assays. Student's t-test (paired two-tailed) and X-2 analyses were used for comparisons between groups. Differences were considered significant at p < 0.05.
Results: BLF501 administration in mice treated with DXR and/or 5-FU decreased the injuries to the mucosa in terms of epithelial integrity and cellular proliferative ability. Co-treatment with BLF501 led to a normal expression and distribution of both zonula occludens-1 (ZO-1) and beta-catenin, which were underexpressed after treatment with either chemotherapeutic agent alone. BLF501 administration also restored normal expression of caspase-3 and ezrin/radixin/moesin (ERM), which were overexpressed after treatment with DXR and 5-FU. In SGLT1-/- mice, BLF501 had no detectable effects. BLF501 administration in wild-type mice with growing A431 tumors did not modify antitumor activity of DXR.
Conclusions: BLF501-induced protection of the intestinal mucosa is a promising novel therapeutic approach to reducing the severity of chemotherapy-induced mucositis.
Gegenstand der Untersuchung ist eine neue Sicherheitsarchitektur, die die Aufgabenzuständigkeiten der Länderpolizei und der Kommune in den zurückliegenden und anzunehmenden künftigen Entwicklungsstufen aufzeigt und verortet. Im Ergebnis wird eine zu starke Stellung der Polizei im Rahmen der vernetzt wahrzunehmenden Aufgabe der Sicherheitsvorsorge vor Ort kritisiert und der Kommune eine Querschnittsaufgabe der Sicherheitsgestaltung im eigenen Wirkungskreis zugeschrieben.
Background: In a 2008-10 study, we found a pretreatment HIV drug resistance (PDR) prevalence of 18.2% in patients at Bugando Medical Centre (BMC) in Mwanza, Tanzania.
Objectives: To determine the prevalence of PDR and transmitted HIV drug resistance (TDR) in patients visiting the BMC from 2013 to 2015.
Methods: Adult outpatients were sequentially enrolled into two groups, separated by whether they were initiating ART. Previous exposure to antiretroviral drugs, except for prevention of mother-to-child transmission, was an exclusion criterion. HIV pol sequences were analysed according to WHO guidelines for surveillance of PDR and TDR.
Results: Two hundred and thirty-five sequences were analysed (138 ART initiators, 97 non-initiators). The prevalence of PDR was 4.7% (95% CI 2.6%-8.2%) overall, 3.1% (95% CI 1.1%-8.7%) for non-initiators and 5.8% (95% CI 3.0%-11.0%) for ART initiators. PDR to NNRTIs and nucleoside or nucelotide reverse transcriptase inhibitors was found in 3.0% (95% CI 1.5%-6.0%) and 1.7% (95% CI 0.7%-4.3%) of patients, respectively. Resistance to PIs was not observed. The prevalence of TDR was 6.0% (95% CI 3.6%-9.8%).
Conclusions: Prevalence of PDR significantly decreased compared with 2008-10 and was below the WHO-defined threshold for triggering a public health response. National and systematic surveillance is needed to inform Tanzania's public health strategy.
Osteoporose ist einer der häufigsten Knochenerkrankungen im fortschreitenden Alter und zählt, aufgrund der damit verbundenen hohen direkten und indirekten Behandlungskosten, zu einer der zehn wichtigsten volkswirtschaftlichen Krankheiten. Die Behandlung der Osteoporose ist langjährig und umfasst eine medikamentöse Therapie auf der Basis einer „knochengesunden“ Lebensweise hinsichtlich Ernährung und Bewegung. Im Rahmen von Untersuchungen zur Linderung von postmenopausalen Beschwerden, zeigte ein Extrakt der Pflanze Cimicifuga racemosa Potential zu osteoprotektiver Wirksamkeit und rückte somit in den Fokus für eine mögliche Anwendung in der Therapie und Prophylaxe von Osteoporose. Das Ziel der vorliegenden Arbeit war es daher, in enger Zusammenarbeit mit der Bionorica SE, welche für die Aufreinigung und Fraktionierung des Pflanzenextraktes zuständig war, und mit der Arbeitsgruppe um Prof. Wuttke, welche parallele Rattenstudien durchführte, Methoden anzuwenden, mit denen osteoprotektive Wirksamkeiten nachgewiesen und auf einzelne Fraktionen des Extraktes limitiert werden können. ...
Background
The allergy preventive effects of gut immune modulation by bacterial compounds are still not fully understood.
Objective
We sought to evaluate the effect of bacterial lysate applied orally from the second until seventh months of life on the prevalence of allergic diseases at school age.
Methods
In a randomized, placebo‐controlled trial, 606 newborns with at least one allergic parent received orally a bacterial lysate consisting of heat‐killed Gram‐negative Escherichia coli Symbio and Gram‐positive Enterococcus faecalis Symbio or placebo from week 5 until the end of month 7. A total of 402 children were followed until school age (6‐11 years) for the assessment of current atopic dermatitis (AD), allergic rhinitis (AR), asthma and sensitization against aeroallergens.
Results
AD was diagnosed in 11.0% (22/200) of children in the active and in 10.4% (21/202) of children in the placebo group. AR was diagnosed in 35% (70/200) of children in the active and in 38.1% (77/202) children in the placebo group. Asthma was diagnosed in 9% (18/199) of children in the active and in 6.6% (13/197) of children in the placebo group. Sensitization occurred in 46.5% (66/142) of participants in the active and 51.7% (76/147) in the placebo group.
Conclusion
An oral bacterial lysate of heat‐killed Gram‐negative Escherichia coli and Gram‐positive Enterococcus faecalis applied during the first 7 months of life did not influence the development of AD, asthma and AR at school age.
Background
Chronic stress is detrimental to health, and children and young people have had to cope with significantly more stress since the start of the COVID-19 pandemic. In particular, stress at school and in relation to learning is a major problem in this age group. Studies in Germany have indicated that the pandemic has led to a reduced quality of life (QoL) and an increased risk for psychiatric disorders in children and adolescents. Schools are an ideal setting for interventions against stress, which is one of the strongest predictors for the development of psychosocial problems. The present study seeks to address stress by means of a short prevention training programme in schools, including emotion regulation, mindfulness, and self-compassion. In addition to information material for self-study, students should have the opportunity to actively deal with the topic of stress and develop coping strategies within a short space of time. In contrast to very long stress reduction programmes that often last several weeks, the programme is delivered in just 90 min.
Methods
The effectiveness of the short and economical prevention programme LessStress will be examined in a cluster-randomised controlled trial (RCT) encompassing 1894 students. At several measurement time points, students from two groups (intervention and control) will be asked about their subjectively perceived stress levels, among other aspects. Due to the clustered nature of the data, mainly multilevel analyses will be performed.
Discussion
In Germany, there are no nationwide universal prevention programmes for students against stress in schools, and this gap has become more evident since the outbreak of the pandemic. Universal stress prevention in schools may be a starting point to promote resilience. By dealing with stress in a healthy way, mental health can be strengthened and maintained. Moreover, to reach at-risk students at an early stage, we advocate for a stronger networking between child psychiatry and schools.
Im Rahmen der Etablierung eines Bettnetzprojektes zur Prävention der Malaria wurden eine städtische (n=60) und eine ländliche Probandengruppe (n=60) in Nigeria im Großraum Abeokuta vor und ein Jahr nach Einführung von mit Insektizid behandelten Bettnetzen (ITN) untersucht. Dabei war das Ziel dieser Arbeit (1) die epidemiologische Malariasituation im Untersuchungsgebiet zu erfassen; (2) Wissen durch Informationsgespräche zu den Themen Prävention, Diagnostik und Therapie der Malaria zu vermitteln; (3), Akzeptanz und Gebrauch der ITN zu dokumentieren; (4), den Effekt von ITN am Verlauf klinischer und labortechnischer Parameter zu verfolgen und (5) die Prävalenz und Höhe von Antikörpern (NANP19) gegen das Circumsporozoiten (CS) Protein, ein Hauptoberflächenprotein der Sporozoiten vor Projektbeginn zu erfassen, sowie ihre Veränderung ein Jahr nach Einführung von ITN. Bei den Untersuchungen zeigte sich, dass Malaria eines der wichtigsten Gesundheitsprobleme der Region ist. Wissensvermittlung förderte die Akzeptanz und den regelmäßigen Gebrauch von ITN. Die klinischen und labortechnischen Untersuchungen bestätigen den protektiven Effekt durch Benutzung von ITN. Fast alle Parameter (Anzahl der Fieberepisoden/Jahr, Anzahl der arbeitsunfähigen Tage, Milzuntersuchung, Ausstrichuntersuchungen und Untersuchung auf Ak) zeigten für die Bettnetzbenutzergruppen Verbesserungen, von denen einige im Vergleich zum Vorjahr hochsignifikant waren. Die Resultate dieser Arbeit weisen darauf hin, dass im hochendemischen Südwesten Nigerias ITN effektiv zur Prävention der Malaria eingesetzt werden können. Der schwere Zugang zu medizinischer Versorgung für ländliche Bevölkerungsgruppen empfiehlt ihren Einsatz besonders in diesen Regionen. Hohe Infektionsraten mit der Gefahr rezidivierender Infektionen, besonders für Kinder, könnten dadurch gesenkt werden. Durch die gezeigte signifikante Reduktion der Krankeheitstage bei regelmäßiger ITN-Benutzung könnte ein zusätzlicher ökonomischer Benefit für Familien sein.
