Background
Up to 50% of septic patients develop acute kidney injury (AKI). The pathomechanism of septic AKI is poorly understood. Therefore, we established an innovative rodent model to characterize sepsis-induced AKI by standardized colon ascendens stent peritonitis (sCASP). The model has a standardized focus of infection, an intensive care set up with monitoring of haemodynamics and oxygenation resulting in predictable impairment of renal function, AKI parameters as well as histopathology scoring.
Methods
Anaesthetized rats underwent the sCASP procedure, whereas sham animals were sham operated and control animals were just monitored invasively. Haemodynamic variables and blood gases were continuously measured. After 24 h, animals were reanesthetized; cardiac output (CO), inulin and PAH clearances were measured and later on kidneys were harvested; and creatinine, urea, cystatin C and neutrophil gelatinase-associated lipocalin (NGAL) were analysed. Additional sCASP-treated animals were investigated after 3 and 9 days.
Results
All sCASP-treated animals survived, whilst ubiquitous peritonitis and significantly deteriorated clinical and macrohaemodynamic sepsis signs after 24 h (MAP, CO, heart rate) were obvious. Blood analyses showed increased lactate and IL-6 levels as well as leucopenia. Urine output, inulin and PAH clearance were significantly decreased in sCASP compared to sham and control. Additionally, significant increase in cystatin C and NGAL was detected. Standard parameters like serum creatinine and urea were elevated and sCASP-induced sepsis increased significantly in a time-dependent manner. The renal histopathological score of sCASP-treated animals deteriorated after 3 and 9 days.
Conclusions
The presented sCASP method is a standardized, reliable and reproducible method to induce septic AKI. The intensive care set up, continuous macrohaemodynamic and gas exchange monitoring, low mortality rate as well as the opportunity of detailed analyses of kidney function and impairments are advantages of this setup. Thus, our described method may serve as a new standard for experimental investigations of septic AKI.
Das Schädel-Hirn-Trauma (SHT) ist ein großes medizinisches Problem. Das Hirnödem mit konsekutiv erhöhten intrakraniellen Drücken ist eine häufige und schwerwiegende Komplikation des schweren SHT. Es ist der signifikanteste Prädiktor für ein schlechtes Outcome. Obwohl Glukokortikoide (GK) die Ausbildung eines Hirnödems bei neuroinflammatorischen Erkrankungen und manchen Hirntumoren reduzieren können, ist diese Substanzklasse im SHT ineffektiv oder sogar schädlich.
Nach controlled cortical impact (CCI) in Mäusen, einem etablierten SHT-Modell in-vivo, zeigte sich ein Zusammenbruch der Blut-Hirn-Schranke (BHS). Des Weiteren wurde der BHS-stabilisierende Effekt der GK nach CCI durch proteasomalen Abbau des Glukokortikoidrezeptors (GR) behindert. Eine Inhibierung des Proteasoms durch den Proteasomeninhibitor Bortezomib zusammen mit einer GK-Therapie mit Dexamethason reduzierte den Abbau des GR und sorgte für eine Restitution der BHS-Integrität.
Unglücklicherweise ließen sich diese Ergebnisse in-vitro nicht auf in Sauerstoff-/Glukosemangel-Modell in der humanen Hirnendothelzelllinie hCMEC/D3 übertragen. Daher konnte für die Kombinationstherapie aus dem Proteasomeninhibitor Bortezomib und Dexamethason kein positiver Effekt gezeigt werden.
Background:
Studies have reported on the incidence of sedation-related adverse events (AEs), but little is known about their impact on health care costs and resource use.
Methods: Health care providers and payers in five countries were recruited for an online survey by independent administrators to ensure that investigators and respondents were blinded to each other. Surveys were conducted in the local language and began with a "screener" to ensure that respondents had relevant expertise and experience. Responses were analyzed using Excel and R, with the Dixon's Q statistic used to identify and remove outliers. Global and country-specific average treatment patterns were calculated via bootstrapping; costs were mean values. The sum product of costs and intervention probability gave a cost per AE.
Results: Responses were received from 101 providers and 26 payers, the majority having. 5 years of experience. At a minimum, the respondents performed a total of 3,430 procedural sedations per month. All AEs detailed occurred in clinical practice in the last year and were reported to cause procedural delays and cancellations in some patients. Standard procedural sedation costs ranged from (sic)74 (Germany) to $2,300 (US). Respondents estimated that AEs would increase costs by between 16% (Italy) and 179% (US). Hypotension was reported as the most commonly observed AE with an associated global mean cost (interquartile range) of $43 ($27-$68). Other frequent AEs, including mild hypotension, bradycardia, tachycardia, mild oxygen desaturation, hypertension, and brief apnea, were estimated to increase health care spending on procedural sedation by $2.2 billion annually in the US.
Conclusion: All sedation-related AEs can increase health care costs and result in substantial delays or cancellations of subsequent procedures. The prevention of even minor AEs during procedural sedation may be crucial to ensuring its value as a health care service.
Rational drug design of Axl tyrosine kinase type I inhibitors as promising candidates against cancer

The high level of Axl tyrosine kinase expression in various cancer cell lines makes it an attractive target for the development of anti-cancer drugs. In this study, we carried out several sets of in silico screening for the ATP-competitive Axl kinase inhibitors based on different molecular docking protocols. The best drug-like candidates were identified, after parental structure modifications, by their highest affinity to the target protein. We found that our newly designed compound R5, a derivative of the R428 patented analog, is the most promising inhibitor of the Axl kinase according to the three molecular docking algorithms applied in the study. The molecular docking results are in agreement with the molecular dynamics simulations using the MM-PBSA/GBSA implicit solvation models, which confirm the high affinity of R5 toward the protein receptor. Additionally, the selectivity test against other kinases also reveals a high affinity of R5 toward ABL1 and Tyro3 kinases, emphasizing its promising potential for the treatment of malignant tumors.
After a large outbreak in Brazil, novel drugs against Zika virus became extremely necessary. Evaluation of virus-based pharmacological strategies concerning essential host factors brought us to the idea that targeting the Axl receptor by blocking its dimerization function could be critical for virus entry. Starting from experimentally validated compounds, such as RU-301, RU-302, warfarin, and R428, we identified a novel compound 2′ (R428 derivative) to be the most potent for this task amongst a number of alternative compounds and leads. The improved affinity of compound 2′ was confirmed by molecular docking as well as molecular dynamics simulation techniques using implicit solvation models. The current study summarizes a new possibility for inhibition of the Axl function as a potential target for future antiviral therapies.
Rund 300 Millionen Alveolen sorgen in der menschlichen Lunge für den Gasaustausch. Eine essentielle Rolle spielen dabei die Alveolarepithelzellen, die die erste Barriere der Luft gegenüber dem Blut bilden. Man unterteilt diese in die kubisch förmigen Typ-II-Epithelzellen, die den wichtigen stabilisierenden Surfactant bilden und in die flacheren Typ-I-Zellen, die aufgrund von engen Zell-Zell Kontakten miteinander eine funktionelle Barriere zwischen Luft und Blut (Blut-Luft-Schranke) ermöglichen. In der Pathophysiologie von verschiedenen Lungenerkrankungen, wie z.B. dem ARDS, spielt die Störung dieser Barriere eine essentielle Rolle.
Ziel der vorliegenden Promotionsarbeit war es, ein in vitro-Modell dieser Barriere, basierend auf der humanen Epithellzelllinie NCI H441, zu etablieren. Die Epithelzellen wurden hierzu erfolgreich auf Transwelleinsätzen gezüchtet.
Um die Barriereeigenschaften zu überprüfen, wurden mit TER Messungen und Fluoreszenzpermeabilitätsmessungen zwei verschiedene Verfahren eingesetzt.
Für die Bewahrung der Konfluenz wurden die Zellen mit Dexamethason – einem potenten Glukokortikoid – behandelt. Dexamethason wurde idealerweise bei jedem Medienwechsel ab Tag 5 apikal hinzugefügt und zeigte einen barrierefördernden Effekt. Die optimale hinzugefügte Dexamethason–konzentration erwies sich als 100 nM. Hierunter bildete die Zellschicht – sowohl mittels TER-Messung als auch mittels Fluoreszenzpermeabilitätsmessung überprüft – eine starke Barriere aus, die am Tag 14-15 nach Zellaussaat ihr Maximum erreichte.
Auf mRNA Ebene konnten zu diesem Zeitpunkt unter 100 nM Dexamethason jeweils relevante Mengen zellspezifischer Proteine von Alveolarepithelzellen Typ-I und II nachgewiesen werden. Mittels Immunfluoreszenzmikroskopie konnte zusätzlich visualisiert werden, dass die zwei Zelltypen koexistieren.
Mittels Immunfluoreszenzmikroskopie zeigte sich weiterhin ein ausgeprägtes Netz an Occludensjunktionsproteinen (Claudin -1,-3,-4, Occludin, ZO-1). In weiteren Versuchen wurde der Einfluß von Endothelzellfaktoren auf das epitheliale Lungenkulturmodell untersucht. Es zeigte sich ein stark ausgeprägter gewebsspezifischer Effekt: Die Faktoren der Lungenendothelzellen stärkten die Barriere des Modells, hingegen die Hirnendothelzellfaktoren sie stark schwächten. Dieser Effekt zeigte sich sowohl durch die gemessenen TER-Werte als auch durch die gemessenen Fluoreszeinpermeabelitätswerte.
Erste Belastungsversuche des Modell durch Hypoxie und reaktive Sauerstoffspezies zeigten eine gewisse Widerstandsfähigkeit der Barriere gegenüber Noxen.
Das vorliegende Zellkulturmodell des Lungenalveolarepithels, basierend auf der NCI H441 Zelllinie, kann folglich genutzt werden, um die Regulation der Barriere genauer zu erforschen und mögliche neue Therapiestrategien zu entwickeln.
Introduction: Distributed ledger networks, chiefly those based on blockchain technologies, currently are heralding a next-generation of computer systems that aims to suit modern users’ demands. Over the recent years, several technologies for blockchains, off-chaining strategies, as well as decentralised and respectively self-sovereign identity systems have shot up so fast that standardisation of the protocols is lagging behind, severely hampering the interoperability of different approaches. Moreover, most of the currently available solutions for distributed ledgers focus on either home users or enterprise use case scenarios, failing to provide integrative solutions addressing the needs of both.
Methods: Herein, we introduce the OpenDSU platform that allows to interoperate generic blockchain technologies, organised–and possibly cascaded in a hierarchical fashion–in domains. To achieve this flexibility, we seamlessly integrated a set of well conceived components that orchestrate off-chain data and provide granularly resolved and cryptographically secure access levels, intrinsically nested with sovereign identities across the different domains. The source code and extensive documentation of all OpenDSU components described herein are publicly available under the MIT open-source licence at https://opendsu.com.
Results: Employing our platform to PharmaLedger, an inter-European network for the standardisation of data handling in the pharmaceutical industry and in healthcare, we demonstrate that OpenDSU can cope with generic demands of heterogeneous use cases in both, performance and handling substantially different business policies.
Discussion: Importantly, whereas available solutions commonly require a predefined and fixed set of components, no such vendor lock-in restrictions on the blockchain technology or identity system exist in OpenDSU, making systems built on it flexibly adaptable to new standards evolving in the future.
Glucocorticoids (GCs) are steroid hormones that have inflammatory and immunosuppressive effects on a wide variety of cells. They are used as therapy for inflammatory disease and as a common agent against edema. The blood brain barrier (BBB), comprising microvascular endothelial cells, serves as a permeability screen between the blood and the brain. As such, it maintains homeostasis of the central nervous system (CNS). In many CNS disorders, BBB integrity is compromised. GC treatment has been demonstrated to improve the tightness of the BBB. The responses and effects of GCs are mediated by the ubiquitous GC receptor (GR). Ligand-bound GR recognizes and binds to the GC response element located within the promoter region of target genes. Transactivation of certain target genes leads to improved barrier properties of endothelial cells. In this review, we deal with the role of GCs in endothelial cell barrier function. First, we describe the mechanisms of GC action at the molecular level. Next, we discuss the regulation of the BBB by GCs, with emphasis on genes targeted by GCs such as occludin, claudins and VE-cadherin. Finally, we present currently available GC therapeutic strategies and their limitations.
In a recent study, we showed in an in vitro murine cerebellar microvascular endothelial cell (cerebEND) model as well as in vivo in rats that Tumor-Treating Fields (TTFields) reversibly open the blood–brain barrier (BBB). This process is facilitated by delocalizing tight junction proteins such as claudin-5 from the membrane to the cytoplasm. In investigating the possibility that the same effects could be observed in human-derived cells, a 3D co-culture model of the BBB was established consisting of primary microvascular brain endothelial cells (HBMVEC) and immortalized pericytes, both of human origin. The TTFields at a frequency of 100 kHz administered for 72 h increased the permeability of our human-derived BBB model. The integrity of the BBB had already recovered 48 h post-TTFields, which is earlier than that observed in cerebEND. The data presented herein validate the previously observed effects of TTFields in murine models. Moreover, due to the fact that human cell-based in vitro models more closely resemble patient-derived entities, our findings are highly relevant for pre-clinical studies.
Despite the availability of numerous therapeutic substances that could potentially target CNS disorders, an inability of these agents to cross the restrictive blood–brain barrier (BBB) limits their clinical utility. Novel strategies to overcome the BBB are therefore needed to improve drug delivery. We report, for the first time, how Tumor Treating Fields (TTFields), approved for glioblastoma (GBM), affect the BBB’s integrity and permeability. Here, we treated murine microvascular cerebellar endothelial cells (cerebEND) with 100–300 kHz TTFields for up to 72 h and analyzed the expression of barrier proteins by immunofluorescence staining and Western blot. In vivo, compounds normally unable to cross the BBB were traced in healthy rat brain following TTFields administration at 100 kHz. The effects were analyzed via MRI and immunohistochemical staining of tight-junction proteins. Furthermore, GBM tumor-bearing rats were treated with paclitaxel (PTX), a chemotherapeutic normally restricted by the BBB combined with TTFields at 100 kHz. The tumor volume was reduced with TTFields plus PTX, relative to either treatment alone. In vitro, we demonstrate that TTFields transiently disrupted BBB function at 100 kHz through a Rho kinase-mediated tight junction claudin-5 phosphorylation pathway. Altogether, if translated into clinical use, TTFields could represent a novel CNS drug delivery strategy.
Progressive deterioration of the central nervous system (CNS) is commonly associated with aging. An important component of the neurovasculature is the blood-brain barrier (BBB), majorly made up of endothelial cells joined together by intercellular junctions. The relationship between senescence and changes in the BBB has not yet been thoroughly explored. Moreover, the lack of in vitro models for the study of the mechanisms involved in those changes impede further and more in-depth investigations in the field. For this reason, we herein present an in vitro model of the senescent BBB and an initial attempt to identify senescence-associated alterations within.
The blood-brain barrier (BBB), made up of endothelial cells of capillaries in the brain, maintains the microenvironment of the central nervous system. During ischemia and traumatic brain injury (TBI), cellular disruption leading to mechanical insult results to the BBB being compromised. Oxygen glucose deprivation (OGD) is the most commonly used in vitro model for ischemia. On the other hand, stretch injury is currently being used to model TBI in vitro. In this paper, the two methods are used alone or in combination, to assess their effects on cerebrovascular endothelial cells cEND in the presence or absence of astrocytic factors. Applying severe stretch and/or OGD to cEND cells in our experiments resulted to cell swelling and distortion. Damage to the cells induced release of lactate dehydrogenase enzyme (LDH) and nitric oxide (NO) into the cell culture medium. In addition, mRNA expression of inflammatory markers interleukin (I L)-6, IL-1\(\alpha\) chemokine (C-C motif) ligand 2 (CCL2) and tumor necrosis factor (TNF)-\(\alpha\) also increased. These events could lead to the opening of calcium ion channels resulting to excitotoxicity. This could be demonstrated by increased calcium level in OGD-subjected cEND cells incubated with astrocyte-conditioned medium. Furthermore, reduction of cell membrane integrity decreased tight junction proteins claudin-5 and occludin expression. In addition, permeability of the endothelial cell monolayer increased. Also, since cell damage requires an increased uptake of glucose, expression of glucose transporter glut1 was found to increase at the mRNA level after OGD. Overall, the effects of OGD on cEND cells appear to be more prominent than that of stretch with regards to TJ proteins, NO, glutl expression, and calcium level. Astrocytes potentiate these effects on calcium level in cEND cells. Combining both methods to model TBI in vitro shows a promising improvement to currently available models.
Die Rolle der B-Linien gestützten Lungensonographie in der Differentialdiagnostik der akuten Dyspnoe

Die akute Dyspnoe ist ein häufiger Aufnahmegrund für Patienten in einer internistischen Notaufnahme. Die für eine schnelle Behandlung notwendige Differenzierung zwischen pulmonalen Ursachen wie einer akut exazerbierten COPD und kardialen Ursachen wie der akuten, dekompensierten Herzinsuffizienz ist dabei initial oftmals nicht sicher möglich. Weit verbreitet in der Diagnostik ist das Röntgenbild des Thorax, welches jedoch einige Zeit in Anspruch nimmt und oftmals nicht unmittelbar nach Aufnahme durchgeführt werden kann. Weitere Instrumente, wie die körperliche Untersuchung und genaue Anamnese sind wichtig, oft aber auch nicht eindeutig. Nicht zuletzt leiden zahlreiche Patienten an mehreren Krankheiten gleichzeitig. Insbesondere bei COPD- Patienten zeigen sich häufig gleichzeitig auch kardiale Symptomatiken.
Eine Möglichkeit zur schnellen Differenzierung zwischen pulmonalen und kardialen Dyspnoeursachen stellt die Lungensonographie mit der Frage nach dem Vorhandensein und der Anzahl von B-Linien dar. Dabei handelt es sich um Ultraschallartefakte, die vermutlich an den Grenzflächen von Wasser und Luft entstehen und ein Hinweis auf Flüssigkeit in der Lunge darstellen. In der hier vorliegenden Untersuchung konnten durch den Autor in der internistischen Notaufnahme der Universitätsklinik Würzburg 39 Patienten, die mit einer Dyspnoesymptomatik aufgenommen wurden, mittels Lungensonographie untersucht werden. Dafür wurde ein kleines, tragbares Gerät mit Sektorschallkopf benutzt. Die Ergebnisse der Lungensonographie wurden mit den Ergebnissen der Röntgenbilder und der Abschlussdiagnose im Entlassbrief verglichen. Es zeigte sich, dass Patienten mit pulmonaler Dyspnoeursache in der Regel keine bzw. nur sehr vereinzelt auftretende B-Linien aufweisen, während kardial dekompensierte Patienten meist zahlreiche und in verschiedenen Schallpositionen vorkommende Artefakte zeigen.
Daneben handelt es sich bei der Lungensonographie um eine leicht zu erlernende, schnell und einfach durchführbare Methode, die schon in der internistischen Notaufnahme direkt beim Patienten anwendbar ist.
Schmerz gehört zu den Kardinalsymptomen einer Entzündung. Im Wesentlichen kann die Entstehung von Schmerz am Ort des Entzündungsgeschehens auf das Einwandern (Diapedese) von Leukozyten aus dem peripheren Blut-strom in das Gewebe zurückgeführt werden. Dort findet sowohl die Produktion von Zytokinen und Chemokinen statt, welche weitere Entzündungszellen rekrutieren und die Entzündungsreaktion verstärken, als auch die Freisetzung von Opioidpeptiden, die schmerzlindernd wirken.
In Vorarbeiten der Arbeitsgruppe konnte eine Opioidfreisetzung aus neutrophilen Granulozyten nach Stimulation mit bakteriellen Antigenen oder Chemokinen \(in\) \(vitro\) nachgewiesen werden. Diese führen \(in\) \(vivo\) eine Antinozizeption herbei. Für neutrophile Granulozyten wurden der Chemokinrezeptor CXCR1/2 sowie der Formylpep-tidrezeptor als Signal-transmittierende Rezeptoren identifiziert. Über den klassischen Mechanismus der Exozytose gelangt das Beta-Endorphin somit in das Gewebe und interagiert mit Opioidrezeptoren auf primär sensorischen Nervenendigungen. \(in\) \(vivo\) äußerte sich die Freisetzung des Opioidpeptids in einer Anhebung mechanischer Schmerzschwellen, die durch den Opioidrezeptorantagonisten Naloxon aufgehoben werden konnten. Die Bindung, vornehmlich an MOP, führt zur Erniedrigung des cAMP-Spiegels, zur Hyperpolarisation der Nervenzelle und zur Verminderung von Schmerzschwellen.
Im Mittelpunkt dieser Arbeit stehen Monozyten als führende Zellpopulation der späten Entzündungsphase. Es sollte untersucht werden, welche Rezeptoren eine Opioidfreisetzung aus Monozyten vermitteln sowie welche intrazellulären Signalwege involviert sind.
Humane Monozyten wurden isoliert und \(in\) \(vitro\) mit dem bakteriellen Antigen Lipopolysaccharide (LPS) stimuliert. Dieses steht exemplarisch für mikrobielles Infektgeschehen und Entzündung. In den Zellüberständen wurde mittels ELISA die Beta-Endorphin-Konzentration ermittelt. Weiterhin wurden Opioidgehalt und -freisetzung in der nicht-klassischen CD14+CD16+ Monozytensubpopulation im Vergleich zu klassischen CD14+CD16- Monozyten analysiert. Zur weiteren Aufklärung des Rezeptors, welcher die Opioidfreisetzung vermittelt, wurde der niedermolekulare TLR4-Antagonist TAK-242 genutzt.
Wir fanden eine Zunahme der Beta-Endorphin-Freisetzung nach Stimulation mit LPS im Vergleich zur unstimulierten Kontrolle. Eine Zugabe des TLR4-Inhibitors reduzierte die Beta-Endorphin-Freisetzung signifikant. TLR4 agiert somit als PRR für die Opioidfreisetzung aus Monozyten. CD14+CD16+ Monozyten enthalten einen geringeren Anteil an Beta-Endorphin und setzten dementsprechend weniger frei. Ihre Rolle als pro-inflammatorisch und ihre Beteiligung an der Genese inflammatorischer Krankheitsbilder wird dadurch gestützt.
Die Signalkaskade, über die diese Freisetzung erfolgt, konnte durch den Einsatz von Rezeptorinhibitoren dahingehend entschlüsselt werden, dass eine Beteiligung des IP3-Rezeptors sowie von intrazellulärem Calcium wichtig ist. Ferner wurde evident, dass auch eine basale Freisetzung existiert, die über denselben Weg verläuft.
Durch die Behandlung mit dem TLR4-Antagonisten TAK-242, der die Freisetzung von Beta-Endorphin \(in\) \(vitro\) unterdrückt, wird auch die analgetische Wirkung von LPS \(in\) \(vivo\) aufgehoben. TLR4 Agonisten sind daher potentielle alternative Analgetika, welche die endogene Schmerzkontrolle unterstützen könnten. Jedoch fließen viele Wechselwirkungen wie z.B. proalgetische Wirkungen von TLR4 in das komplexe Gefüge der Immunzellantwort ein. Diese wurden nicht weiter untersucht. Vor einer klinischen Anwendung müssten solche Effekte näher betrachtet werden.
Pilotstudie zur Evaluation eines minimal-invasiven Diagnoseverfahrens der Malignen Hyperthermie

Die Maligne Hyperthermie ist eine genetisch determinierte Stoffwelchselerkrankung, bei der es unter der Exposition von volatilen Anästhetika und depolarisierenden Muskelrelaxantien zu einem generalisierten Hypermetabolismus mit Laktatazidose und vermehrter Produktion von CO2 kommt. Die Diagnose wird mittels dem, für den Patienten belastenden, invasiven Invitro-Kontrakturtest als Goldstandard gestellt. Ein wichtiger Schwerpunkt der anästhesiologischen Forschung liegt daher in der Entwicklung eines weniger invasiven Diagnoseverfahrens der Disposition zur Malignen Hyperthermie. Als Modell der malignen Hyperthermie ist das „porcine-stress-syndrom“ etabliert. Wir nahmen an, dass durch die intramuskuläre Applikation von Koffein und Halothan als Triggersubstanzen bei stressempfindlichen Schweinen (MHS) im Gegensatz zu nicht veranlagten Tieren (MHN) eine lokal begrenzte hypermetabole Reaktion ohne systemische Auswirkungen ausgelöst wird. Nach Genehmigung durch die lokale Ethikkomission wurden 4 Schweine mit (MHS) und 3 Schweine (MHN) ohne Veranlagung für das „porcin-stress-syndrom“ jeweils 6-mal in Narkose untersucht. Es wurden 3 modifizierte Mikrodialyse (MD) - Sonden, sowie eine pH- / pCO2- Messsonde, in die Adduktorenmuskulatur der Tiere eingebracht. Bei der ersten MD - Sonde wurde ein Schlauch zur Applikation von Koffeinlösungen angebracht. In 2 Vorversuchen wurden nach Äquilibrierung steigende Volumina (50, 100, 200 und 400 µl) von 80 bzw. 20 mM zugegeben. Im Hauptversuch wurden 100µl Koffeinlösung ansteigender Konzentration (10, 20 und 40 mM) verwendet. Bei der zweiten MD - Sonde handelte es sich um eine Mess- und eine Stimulationssonde, welche mit einer 10%-igen Lösung von Halothan in Lipofundin 20% bei 1 µl/min perfundiert wurde. Die dritte MD - Sonde lieferte Kontrollwerte. Das Dialysat wurde bei einer Perfusionsgeschwindigkeit von 2 µl/min in 15-minütigen Intervallen aufgefangen und die Laktatkonzentration photometrisch bestimmt. In der Mitte der pH- / pCO2- Messsonde wurde im Vorversuch 20 mM Koffein steigender Volumina (50, 100, 200 und 400 µl), im Hauptversuch 100 µl Koffein steigender Konzentration (2, 5, 10 und 20 mM) injiziert und der pH /pCO2 kontinuierlich gemessen. Bei Versuchsdurchführung wurden hämodynamische und metabolische Parameter dokumentiert. Nach dem letzten Versuch wurde eine Maligne-Hyperthermie-Krise ausgelöst, sowie die Zugehörigkeit zu den Versuchgruppen mittels Invitro-Kontrakturtest und genetischer Testung gesichert. Im Rahmen dieser Pilotstudie wurde bei einer nur geringen Anzahl unabhängiger Versuche auf eine statistische Auswertung verzichtet. Die Einzelversuche wurden im Detail ausgewertet. Die Ergebnisse der Mikrodialyse-Versuche unter Stimulation mit Koffein erbrachten bei Anlageträgern nur bei maximaler Triggerung mit einem 20 mM bzw. 80 mM Koffeinbolus einen messbaren Laktat-anstieg. Eine Annäherung an diese Schwellendosis, ausgehend von niedrigeren Boluskonzentrationen, zeigte keine messbaren Veränderungen der lokalen Laktatkonzentration. Unter der kontinuierlichen Stimulation mit Halothan findet sich nur bei den MHS-Tieren ein Anstieg der Laktatkonzentration. Die Werte der intraindividuellen Kontrollsonden unterschieden sich dabei nicht in den Gruppen der MHS- / MHN-Tiere. Die Messung des Gewebe-pH / -pCO2 im Vorversuch erbrachte bereits durch den initialen 20 mM Koffeinbolus bei den MHS-Tieren einen ausgeprägten Abfall des lokalen pH-Wertes und Anstieg des pCO2 im Sinne eines lokalen Hypermetabolismus. Der Hauptversuch zeigte, dass im Gegensatz zu den MHN-Tieren bei repetitiver Stimulation nur in der Gruppe der MHS-Tiere ab einer Koffeinkonzentration von 5 mM ein zunehmender Abfall des lokalen pH-Wertes und ein Anstieg des pCO2 über das Ausgangsniveau auszulösen ist. Klinisch zeigte sich keine systemische Reaktion auf die lokale Triggerapplikation. Ebenso wurden keine Unterschiede in den hämodynamischen und metabolischen Parameter der MHS- / MHN-Tiere gesehen. Im Rahmen der vorliegenden Untersuchung konnte gezeigt werden, dass es in-vivo möglich ist bei MH-Anlageträgern, mittels intramuskulär applizierter Triggersubstanzen, einen lokalen, MH-ähnlichen Hypermetabolismus auszulösen ohne Induktion einer systemischen Reaktion. Auf Grund dieser Erkenntnisse konnte eine Übertragung des Testverfahrens auf den Menschen erfolgen, welches das Potenzial hat, den wesentlich invasiveren In-vitro-Kontrakturtest als Diagnoseverfahren abzulösen.
Ischemic stroke is one of the leading causes of death worldwide. It damages neurons and other supporting cellular elements in the brain. However, the impairment is not only confined to the region of assault but the surrounding area as well. Besides, it also brings about damage to the blood-brain barrier (BBB) which in turn leads to microvascular failure and edema. Hence, this necessitates an on-going, continuous search for intervention strategies and effective treatment. Of late, the natural sweetener stevioside proved to exhibit neuroprotective effects and therapeutic benefits against cerebral ischemia-induced injury. Its injectable formulation, isosteviol sodium (STVNA) also demonstrated favorable results. Nonetheless, its effects on the BBB have not yet been investigated to date. As such, this present study was designed to assess the effects of STVNA in our in vitro stroke model of the BBB.The integrity and permeability of the BBB are governed and maintained by tight junction proteins (TJPs) such as claudin-5 and occludin. Our data show increased claudin-5 and occludin expression in oxygen and glucose (OGD)-deprived murine brain capillary cerebellar endothelial cells (cerebEND) after STVNa treatment. Likewise, the upregulation of the transmembrane protein integrin-αv was also observed. Finally, cell volume was reduced with the simultaneous administration of STVNA and OGD in cerebEND cells. In neuropathologies such as stroke, the failure of cell volume control is a major feature leading to loss of cells in the penumbra as well as adverse outcomes. Our initial findings, therefore, point to the neuroprotective effects of STVNA at the BBB in vitro, which warrant further investigation for a possible future clinical intervention.
Ziel dieser Arbeit war der Nachweis eines neuroprotektiven Effektes von STVNA auf cerebEND Zellen der Maus in einem in vitro Modell des Schlaganfalls. Mit dem Verfahren zur Herstellung von STVNA konnte ein reines und im Vergleich zu Isosteviol in Wasser gut lösliches Produkt hergestellt werden, das die Anforderungen an eine Versuchssubstanz in einem in vitro Modell voll erfüllen konnte. Als in vitro Modell wurde das bereits bewährte Verfahren der OGD gewähl. CerebEND Zellen der Maus wurden für 4 h OGD ausgesetzt und anschließend für 4 h und 24 h mit 0, 1, 5, 10 und 20 mg/l STVNA behandelt. Direkt, 4 h und 24 h nach 4 h OGD wurden die jeweiligen Zellen geerntet und mittels Western Blot und qRT-PCR ausgewertet. Es wurden eine erhöhte Expression der Tight-Junction-Proteine Claudin-5 und Occludin, sowie ein stabilisierendes Expressionsverhalten der Transmembranproteine Integrin a 1 und Integrin a v nach Behandlung mit STVNA nachgewiesen. Ebenso wurde eine verminderte Expression des Glukosetransporters GLUT 1 beobachtet. Eine Volumenreduktion der cerebEND Zellen durch STVNA, während 4h OGD und gleichzeitiger Behandlung mit STVNA konnte ebenfalls festgestellt werden. Die Ergebnisse dieser Arbeit stützen die Thesen und Ergebnisse der aktuellen Literatur, dass STVNA neuroprotektive Eigenschaften hat.
Background: The German-language recommendations for the management of postoperative nausea and vomiting(PONV) have been revised by an expert committee. Major aspects of this revision are presented here in the form of an evidence-based review article.
Methods: The literature was systematically reviewed with the goal of revising the existing recommendations. New evidence-based recommendations for the management of PONV were developed, approved by consensus, and graded according to the scheme of the Scottish Intercollegiate Guidelines Network (SIGN).
Results: The relevant risk factors for PONV include female sex, nonsmoker status, prior history of PONV, motion sickness, use of opioids during and after surgery, use of inhalational anesthetics and nitrous oxide, and the duration of anesthesia. PONV scoring systems provide a rough assessment of risk that can serve as the basis for a riskadapted approach. Risk-adapted prophylaxis, however, has not been shown to provide any greater benefit than fixed (combination) prophylaxis, and PONV risk scores have inherent limitations; thus, fixed prophylaxis may be advantageous. Whichever of these two approaches to manage PONV is chosen, high-risk patients must be given multimodal prophylaxis, involving both the avoidance of known risk factors and the application of multiple validated and effective antiemetic interventions. PONV should be treated as soon as it arises, to minimize patient discomfort, the risk of medical complications, and the costs involved.
Conclusion: PONV lowers patient satisfaction but is treatable. The effective, evidence-based measures of preventing and treating it should be implemented in routine practice.
Die patientenkontrollierte Analgesie (PCA) hat sich in den letzten Jahren als Therapieoption bei der postoperativen Schmerztherapie etabliert. Dabei gibt es zwei Verfahren, die intravenöse (PCIA) und die epidurale patientenkontrollierte Analgesie (PCEA). Ziel der vorliegenden Arbeit war es, zu untersuchen, ob die PCA eine effiziente postoperative Schmerztherapie bieten kann und ob eines der beiden Verfahren besser abschneidet. Neben dem für die Therapie notwendigen Aufwand sollten auch die Risiken betrachtet werden. Die Patienten kamen aus den Kliniken der Urologie, der Allgemein- und der Herz-Thorax-Chirurgie, sie hatten alle einen größeren operativen Eingriff hinter sich. Die Operationsarten wurden eingeteilt in Thorax, Oberbauch, Colon, urologische und Sonstiges. Die Thorax-Patienten wurden ausschließlich mit PCIA versorgt, bei den Oberbauch-, den Colon- und den Urologie-Patienten überwog das PCEA-Verfahren. Der durchschnittliche tägliche Zeitaufwand für die Visiten alleine lag bei etwa 80 Minuten. Für alle Visiten eines durchschnittlichen PCEA-Patienten wurde ungefähr dreimal soviel Zeit benötigt wie für die eines PCIA-Patienten. Insgesamt wurden täglich im Schnitt um die 110 Minuten für die patientenkontrollierte Analgesie benötigt. Die Kosten für die Analgetika waren mit 150 Euro beim PCEA-Verfahren etwa fünf mal so hoch wie beim PCIA-Verfahren. Zur Erfassung der Effizienz der Schmerztherapie wurden die Visuelle Analogskala (VAS), die Differenz der geforderten und erhaltenen Patientenboli sowie die Angaben, in wie vielen Visiten eine zusätzliche Analgesie notwendig war, herangezogen. Insgesamt waren die Patienten mit beiden Verfahren suffizient analgesiert. Im Vergleich zeigten sich jedoch bei dem PCEA-Verfahren deutlich niedrigere VAS-Werte. Außerdem benötigten die PCEA-Patienten in nur 25% aller Visiten eine zusätzliche Analgesie, im Gegensatz zu den PCIA-Patienten, die in über 50% eine zusätzliche Schmerzmedikation bekamen. Auch bei der Differenz der geforderten und erhaltenen Patientenboli schnitt das PCEA-Verfahren besser ab. Als unerwünschte Wirkungen traten bei den PCIA-Patienten überwiegend Übelkeit und Sedierung, in der PCEA-Gruppe Parästhesien auf. Ein PCEA-Patient mußte neurochirurgisch versorgt werden, nachdem es bei ihm zu einem epiduralen Abszeß gekommen war. Die PCA ist ein suffizientes Verfahren in der postoperativen Schmerztherapie. Sowohl PCIA als auch PCEA bieten eine ausreichende Analgesie, jedoch wird durch die PCEA, bei größerem Aufwand, eine bessere Analgesie erreicht. Die Risiken sind überschaubar, setzen jedoch ein regelmäßiges Monitoring voraus.
Die Maligne Hyperthermie ist eine latente metabolische Myopathie, die durch Exposition mit volatilen Anästhetika oder depolarisierenden Muskelrelaxantien in disponierten Individuen zu einem potentiell lebensbedrohlichen hypermetabolen Syndrom der Skelettmuskulatur führen kann. Dieser Zustand basiert auf einer unkontrollierten sarkoplasmatischen Kalziumfreisetzung über funktionell veränderte Ryanodinrezeptoren. Die klinische Symptomatik umfasst einen Anstieg des Kohlendioxidpartialdrucks und der Körperkerntemperatur sowie eine Tachykardie, Laktatazidose und Muskelspasmen. Zur Diagnostik einer MH-Veranlagung stellt der In-Vitro-Kontrakturtest bis heute das einzige verlässliche diagnostische Verfahren dar. Der in dieser Studie untersuchte metabolische Test bietet aufgrund seiner minimalen Invasivität und seinem geringeren zeitlichen bzw. kostenintensiven Aufwand relevante Vorteile gegenüber dem In-Vitro-Kontrakturtest. In dieser Studie wurde untersucht, ob unter Einsatz der MAB 7 Mikrodialysesonden mit integriertem Zuspritzkatheter der intramuskuläre Laktatspiegel durch lokale Applikation von Halothan 4 Vol% und Koffein 80 mM gesteigert werden kann und somit eine Differenzierung zwischen MHS- und MHN-Individuen möglich ist. Mit Genehmigung der örtlichen Ethikkommission wurden bei 7 MHS-, 9 MHN-, 7 MHN-Neuro- und 7 Kontrollprobanden je vier Mikrodialysesonden im Musculus vastus lateralis platziert. Nach 20-minütiger Äquilibrierungszeit wurden über je 2 Sonden 200 µl Koffein 80 mM bzw. 200 µl Halothan gelöst in Sojabohnenöl mit einer Flussgeschwindigkeit von 70 µl/min injiziert und die Laktatkonzentration im Dialysat spektrophotometrisch gemessen. Sowohl nach Stimulation mit Halothan 4 Vol% als auch Koffein 80 mM kam es in der MHS-Gruppe zu einem signifikanten Anstieg der Laktatkonzentrationen im Vergleich zu allen drei anderen Probandengruppen. Ebenso waren die erreichten Maximalwerte der MHS-Probanden signifikant höher verglichen mit denen der MHN-, MHN-Neuro- und Kontrollgruppen. Als Zeichen der stärker abgelaufenen Stoffwechselreaktion waren die Kreatinkinase und die VAS-Werte nach Triggerapplikation in der MHS-Gruppe signifikant erhöht. Systemische hämodynamische und metabolische Parameter blieben bei allen vier Probandengruppen im Normbereich. Wie schon in vorangegangenen Untersuchungen gezeigt, belegt diese Studie, dass die intramuskuläre Stimulation mit MH-Triggersubstanzen zu einer Aktivierung der lokalen Stoffwechselvorgänge führt mit einem signifikanten Laktatanstieg in MH-disponierten Individuen. Die gewählten Triggerkonzentrationen von Halothan 4 Vol% und Koffein 80 mM ermöglichten eine Unterscheidung zwischen MHS- und MHN-Probanden. Es kam jedoch bei drei nicht MH-veranlagten Probanden zu einem falsch-positiven Ergebnis. Die erstmalig eingesetzten MAB 7 Mikrodialysesonden mit integriertem Zuspritzkatheter zeigten konstante relative Recovery-Werte unter den jeweiligen Flussgeschwindigkeiten. Die Einführung dieser Sonden stellt einen weiteren Schritt zur Vereinfachung und Standardisierung des Verfahrens dar. Durch weitere methodische Untersuchungen mit variierenden Parametern wie Trigger-konzentration, Menge und Applikationsintervall scheint es möglich, noch eindeutigere Grenzen zur sicheren Unterscheidung zwischen MHS und MHN ziehen zu können. In dieser Studie konnte gezeigt werden, dass die Entwicklung des minimal-invasiven Testverfahrens eine für die Zukunft vielversprechende klinische Grundlage zur Diagnostik einer MH-Veranlagung darstellt.
Background
Enteric glial cells (EGCs) are the main constituent of the enteric nervous system and share similarities with astrocytes from the central nervous system including their reactivity to an inflammatory microenvironment. Previous studies on EGC pathophysiology have specifically focused on mucosal glia activation and its contribution to mucosal inflammatory processes observed in the gut of inflammatory bowel disease (IBD) patients. In contrast knowledge is scarce on intestinal inflammation not locally restricted to the mucosa but systemically affecting the intestine and its effect on the overall EGC network.
Methods and Results
In this study, we analyzed the biological effects of a systemic LPS-induced hyperinflammatory insult on overall EGCs in a rat model in vivo, mimicking the clinical situation of systemic inflammation response syndrome (SIRS). Tissues from small and large intestine were removed 4 hours after systemic LPS-injection and analyzed on transcript and protein level. Laser capture microdissection was performed to study plexus-specific gene expression alterations. Upon systemic LPS-injection in vivo we observed a rapid and dramatic activation of Glial Fibrillary Acidic Protein (GFAP)-expressing glia on mRNA level, locally restricted to the myenteric plexus. To study the specific role of the GFAP subpopulation, we established flow cytometry-purified primary glial cell cultures from GFAP promotor-driven EGFP reporter mice. After LPS stimulation, we analyzed cytokine secretion and global gene expression profiles, which were finally implemented in a bioinformatic comparative transcriptome analysis. Enriched GFAP+ glial cells cultured as gliospheres secreted increased levels of prominent inflammatory cytokines upon LPS stimulation. Additionally, a shift in myenteric glial gene expression profile was induced that predominantly affected genes associated with immune response.
Conclusion and Significance
Our findings identify the myenteric GFAP-expressing glial subpopulation as particularly susceptible and responsive to acute systemic inflammation of the gut wall and complement knowledge on glial involvement in mucosal inflammation of the intestine.
The interplay of specific leukocyte subpopulations, resident cells and proalgesic mediators results in pain in inflammation. Proalgesic mediators like reactive oxygen species (ROS) and downstream products elicit pain by stimulation of transient receptor potential (TRP) channels. The contribution of leukocyte subpopulations however is less clear. Local injection of neutrophilic chemokines elicits neutrophil recruitment but no hyperalgesia in rats. In meta-analyses the monocytic chemoattractant, CCL2 (monocyte chemoattractant protein-1; MCP-1), was identified as an important factor in the pathophysiology of human and animal pain. In this study, intraplantar injection of CCL2 elicited thermal and mechanical pain in Wistar but not in Dark Agouti (DA) rats, which lack p47phox, a part of the NADPH oxidase complex. Inflammatory hyperalgesia after complete Freund's adjuvant (CFA) as well as capsaicin-induced hyperalgesia and capsaicin-induced current flow in dorsal root ganglion neurons in DA were comparable to Wistar rats. Macrophages from DA expressed lower levels of CCR2 and thereby migrated less towards CCL2 and formed limited amounts of ROS in vitro and 4-hydroxynonenal (4-HNE) in the tissue in response to CCL2 compared to Wistar rats. Local adoptive transfer of peritoneal macrophages from Wistar but not from DA rats reconstituted CCL2-triggered hyperalgesia in leukocyte-depleted DA and Wistar rats. A pharmacological stimulator of ROS production (phytol) restored CCL2-induced hyperalgesia in vivo in DA rats. In Wistar rats, CCL2-induced hyperalgesia was completely blocked by superoxide dismutase (SOD), catalase or tempol. Likewise, inhibition of NADPH oxidase by apocynin reduced CCL2-elicited hyperalgesia but not CFA-induced inflammatory hyperalgesia. In summary, we provide a link between CCL2, CCR2 expression on macrophages, NADPH oxidase, ROS and the development CCL2-triggered hyperalgesia, which is different from CFA-induced hyperalgesia. The study further supports the impact of CCL2 and ROS as potential targets in pain therapy.
CXCL10 Controls Inflammatory Pain via Opioid Peptide- Containing Macrophages in Electroacupuncture

Acupuncture is widely used for pain treatment in patients with osteoarthritis or low back pain, but molecular mechanisms remain largely enigmatic. In the early phase of inflammation neutrophilic chemokines direct opioid-containing neutrophils in the inflamed tissue and stimulate opioid peptide release and antinociception. In this study the molecular pathway and neuroimmune connections in complete Freund's adjuvant (CFA)-induced hind paw inflammation and electroacupuncture for peripheral pain control were analyzed. Free moving Wistar rats with hind paw inflammation were treated twice with electroacupuncture at GB30 (Huan Tiao - gall bladder meridian) (day 0 and 1) and analyzed for mechanical and thermal nociceptive thresholds. The cytokine profiles as well as the expression of opioid peptides were quantified in the inflamed paw. Electroacupuncture elicited long-term antinociception blocked by local injection of anti-opioid peptide antibodies (beta-endorphin, met-enkephalin, dynorphin A). The treatment altered the cytokine profile towards an anti-inflammatory pattern but augmented interferon (IFN)-gamma and the chemokine CXCL10 (IP-10: interferon gamma-inducible protein) protein and mRNA expression with concomitant increased numbers of opioid peptide-containing CXCR3+ macrophages. In rats with CFA hind paw inflammation without acupuncture repeated injection of CXCL10 triggered opioid-mediated antinociception and increase opioid-containing macrophages. Conversely, neutralization of CXCL10 time-dependently decreased electroacupuncture-induced antinociception and the number of infiltrating opioid peptide-expressing CXCR3+ macrophages. In summary, we describe a novel function of the chemokine CXCL10 - as a regulator for an increase of opioid-containing macrophages and antinociceptive mediator in inflammatory pain and as a key chemokine regulated by electroacupuncture.
Background
Leukocytes containing opioid peptides locally control inflammatory pain. In the early phase of complete Freund’s adjuvant (CFA)-induced hind paw inflammation, formyl peptides (derived e.g. from Mycobacterium butyricum) trigger the release of opioid peptides from neutrophils contributing to tonic basal antinociception. In the later phase we hypothesized that toll-like-receptor-(TLR)-4 activation of monocytes/macrophages triggers opioid peptide release and thereby stimulates peripheral opioid-dependent antinociception.
Results
In Wistar rats with CFA hind paw inflammation in the later inflammatory phase (48–96 h) systemic leukocyte depletion by cyclophosphamide (CTX) or locally injected naloxone (NLX) further decreased mechanical and thermal nociceptive thresholds. In vitro β-endorphin (β-END) content increased during human monocyte differentiation as well as in anti-inflammatory CD14+CD16- or non-classical M2 macrophages. Monocytes expressing TLR4 dose-dependently released β-END after stimulation with lipopolysaccharide (LPS) dependent on intracellular calcium. Despite TLR4 expression proinflammatory M1 and anti-inflammatory M2 macrophages only secreted opioid peptides in response to ionomycin, a calcium ionophore. Intraplantar injection of LPS as a TLR4 agonist into the inflamed paw elicited an immediate opioid- and dose-dependent antinociception, which was blocked by TAK-242, a small-molecule inhibitor of TLR4, or by peripheral applied NLX. In the later phase LPS lowered mechanical and thermal nociceptive thresholds. Furthermore, local peripheral TLR4 blockade worsened thermal and mechanical nociceptive pain thresholds in CFA inflammation.
Conclusion
Endogenous opioids from monocytes/macrophages mediate endogenous antinociception in the late phase of inflammation. Peripheral TLR4 stimulation acts as a transient counter-regulatory mechanism for inflammatory pain in vivo, and increases the release of opioid peptides from monocytes in vitro. TLR4 antagonists as new treatments for sepsis and neuropathic pain might unexpectedly transiently enhance pain by impairing peripheral opioid analgesia.
Background: Selective outcome reporting in clinical trials introduces bias in the body of evidence distorting clinical decision making. Trial registration aims to prevent this bias and is suggested by the International Committee of Medical Journal Editors (ICMJE) since 2004.
Methods: The 585 randomized controlled trials (RCTs) published between 1965 and 2017 that were included in a recently published Cochrane review on antiemetic drugs for prevention of postoperative nausea and vomiting were selected. In a retrospective study, we assessed trial registration and selective outcome reporting by comparing study publications with their registered protocols according to the ‘Cochrane Risk of bias’ assessment tool 1.0.
Results: In the Cochrane review, the first study which referred to a registered trial protocol was published in 2004. Of all 585 trials included in the Cochrane review, 334 RCTs were published in 2004 or later, of which only 22% (75/334) were registered. Among the registered trials, 36% (27/75) were pro- and 64% (48/75) were retrospectively registered. 41% (11/27) of the prospectively registered trials were free of selective outcome reporting bias, 22% (6/27) were incompletely registered and assessed as unclear risk, and 37% (10/27) were assessed as high risk. Major outcome discrepancies between registered and published high risk trials were a change from the registered primary to a published secondary outcome (32%), a new primary outcome (26%), and different outcome assessment times (26%). Among trials with high risk of selective outcome reporting 80% favoured at least one statistically significant result. Registered trials were assessed more often as ‘overall low risk of bias’ compared to non-registered trials (64% vs 28%).
Conclusions: In 2017, 13 years after the ICMJE declared prospective protocol registration a necessity for reliable clinical studies, the frequency and quality of trial registration in the field of PONV is very poor. Selective outcome reporting reduces trustworthiness in findings of clinical trials. Investigators and clinicians should be aware that only following a properly registered protocol and transparently reporting of predefined outcomes, regardless of the direction and significance of the result, will ultimately strengthen the body of evidence in the field of PONV research in the future.
Im Jahr 2017, 13 Jahre nachdem das ICMJE die prospektive Protokollregistrierung zur Notwendigkeit für zuverlässige klinische Studien erklärt hat, ist die Häufigkeit und Qualität der Studienregistrierung im Bereich der PONV-Forschung sehr gering. Für nur ein Fünftel der seit dem Jahr 2004 publizierten klinischen Studien, die in den 2020 veröffentlichten PONV Cochrane Review aufgenommen wurden, wurden Studienprotokolle registriert. Von diesen waren fast zwei Drittel retrospektiv registriert. Schlussendlich konnten weniger als 50% der prospektiv registrierten Studien als frei von Bias bei der selektiven Endpunktberichterstattung identifiziert werden. Dies ist ein alarmierendes Defizit. Diese Arbeit zeigt auch, dass registrierte Studien im Allgemeinen häufiger mit einem niedrigen Gesamtbiasrisiko beurteilt wurden. Dies legt die Studienregistrierung als Qualitätskriterium für RCTs in der klinischen PONV-Forschung nahe. Bias durch selektive Endpunktberichterstattung verringert die Vertrauenswürdigkeit von Studienergebnissen. Wissenschaftler*innen und Kliniker*innen sollten sich darüber im Klaren sein, dass nur die Adhärenz bezüglich einer adäquaten Protokollregistrierung und die transparente Berichterstattung über vordefinierte Endpunkte, unabhängig von Richtung und Bedeutung deren Ergebnisse, letztlich die Evidenz der PONV-Forschung in der Zukunft stärken kann.
Ischämische Postkonditionierung beschreibt in der Kardioprotektion ein Phänomen bei dem kurze Wechsel von Reperfusion und Ischämie zu Beginn der Wiedereröffnung eines verschlossenen Gefäßes dazu führen den Ischämie/Reperfusions-Schaden zu begrenzen. Verschiedene Kaliumkanäle in der inneren mitochondrialen Membran sind in aktiviertem Zustand an der Kardioprotektion durch Konditionierung beteiligt. Die Rolle des Calcium-aktivierten Kalium-Kanals mit großer Leitfähigkeit (BKCa) in der ischämischen Postkonditionierung wurde bisher nicht untersucht. In der vorliegenden Arbeit wurde die Hypothese getestet, dass die ischämische Postkonditionierung durch den BKCa vermittelt wird. Derzeit wird die mPTP als wichtigster Endeffektor der Kardioprotektion in Prä- und Postkonditionierung beschrieben. Als weitere Hypothese wurde überprüft, ob die mPTP dem BKCa in der Signalvermittlung der ischämischen Postkonditionierung nachgeschaltet ist.
Die Experimente wurden an einem etablierten in-vivo-Myokardinfarktmodell der Maus durchgeführt. Männliche C57BL6-Mäuse wurden randomisiert auf 13 verschiedene Gruppen verteilt. In den einzelnen Gruppen wurden IPOST sowie Aktivatoren und Inhibitoren von BKCa und mPTP in verschiedensten Kombinationen verabreicht. Es wurden die Infarktgrößen in den Gruppen bestimmt und miteinander verglichen.
Es konnte gezeigt werden, dass der BKCa an der Kardioprotektion durch IPOST beteiligt ist. Die Blockade des BKCa hob die Effekte der IPOST auf. Auch die mPTP ist an der IPOST beteiligt. Durch Öffnung der mPTP mittels Atractyloside wurde die IPOST aufgehoben. Trotz der Blockade des BKCa durch Iberiotoxin führte CYC A in der Kombination IbTx+CYC A zur Infarktgrößenreduktion. In der Gruppe NS1619+ATRA fand sich eine Infarktgrößenreduktion gegenüber CON und ATRA.
Insgesamt zeigt sich, dass IPOST kardioprotektiv ist und über BKCa und mPTP vermittelt wird. Anhand der vorliegenden Ergebnisse kann eine mögliche Interaktion von BKCa und mPTP nicht abschließend beurteilt werden.
Early treatment with glucocorticoids could help reduce both cytotoxic and vasogenic edema, leading to improved clinical outcome after stroke. In our previous study, isosteviol sodium (STVNA) demonstrated neuroprotective effects in an in vitro stroke model, which utilizes oxygen-glucose deprivation (OGD). Herein, we tested the hypothesis that STVNA can activate glucocorticoid receptor (GR) transcriptional activity in brain microvascular endothelial cells (BMECs) as previously published for T cells. STVNA exhibited no effects on transcriptional activation of the glucocorticoid receptor, contrary to previous reports in Jurkat cells. However, similar to dexamethasone, STVNA inhibited inflammatory marker IL-6 as well as granulocyte-macrophage colony-stimulating factor (GM-CSF) secretion. Based on these results, STVNA proves to be beneficial as a possible prevention and treatment modality for brain ischemia-reperfusion injury-induced blood–brain barrier (BBB) dysfunction.
Background: COVID-19 patients are at high thrombotic risk. The safety and efficacy of different anticoagulation regimens in COVID-19 patients remain unclear. Methods: We searched for randomised controlled trials (RCTs) comparing intermediate- or therapeutic-dose anticoagulation to standard thromboprophylaxis in hospitalised patients with COVID-19 irrespective of disease severity. To assess efficacy and safety, we meta-analysed data for all-cause mortality, clinical status, thrombotic event or death, and major bleedings. Results: Eight RCTs, including 5580 patients, were identified, with two comparing intermediate- and six therapeutic-dose anticoagulation to standard thromboprophylaxis. Intermediate-dose anticoagulation may have little or no effect on any thrombotic event or death (RR 1.03, 95% CI 0.86–1.24), but may increase major bleedings (RR 1.48, 95% CI 0.53–4.15) in moderate to severe COVID-19 patients. Therapeutic-dose anticoagulation may decrease any thrombotic event or death in patients with moderate COVID-19 (RR 0.64, 95% CI 0.38–1.07), but may have little or no effect in patients with severe disease (RR 0.98, 95% CI 0.86–1.12). The risk of major bleedings may increase independent of disease severity (RR 1.78, 95% CI 1.15–2.74). Conclusions: Certainty of evidence is still low. Moderately affected COVID-19 patients may benefit from therapeutic-dose anticoagulation, but the risk for bleeding is increased.

Neuropathic pain, caused by neuronal damage, is a severely impairing mostly chronic condition. Its underlying molecular mechanisms have not yet been thoroughly understood in their variety. In this doctoral thesis, I investigated the role of microRNAs (miRNAs) in a murine model of peripheral neuropathic pain. MiRNAs are small, non-coding RNAs known to play a crucial role in post-transcriptional gene regulation, mainly in cell proliferation and differentiation. Initially, expression patterns in affected dorsal root ganglia (DRG) at different time points after setting a peripheral nerve lesion were studied. DRG showed an increasingly differential expression pattern over the course of one week. Interestingly, a similar effect, albeit to a smaller extent, was observed in corresponding contralateral ganglia. Five miRNA (miR-124, miR-137, miR-183, miR-27b, and miR-505) were further analysed. qPCR, in situ hybridization, and bioinformatical analysis point towards a role for miR-137 and -183 in neuropathic pain as both were downregulated. Furthermore, miR-137 is shown to be specific for non-peptidergic non-myelinated nociceptors (C fibres) in DRG. As the ganglia consist of highly heterocellular tissue, I also developed a neuron-specific approach. Primarily damaged neurons were separated from intact adjacent neurons using fluorescence-activated cell-sorting and their gene expression pattern was analysed using a microarray. Thereby, not only were information obtained about mRNA expression in both groups but, by bioinformatical tools, also inferences on miRNA involvement. The general expression pattern was consistent with previous findings. Still, several genes were found differentially expressed that had not been described in this context before. Among these are corticoliberin or cation-regulating proteins like Otopetrin1. Bioinformatical data conformed, in part, to results from whole DRG, e.g. they implied a down-regulation of miR-124, -137, and -183. However, these results were not significant.
In summary, I found that a) miRNA expression in DRG is influenced by nerve lesions typical of neuropathic pain and that b) these changes develop simultaneously to over-expression of galanin, a marker for neuronal damage. Furthermore, several miRNAs (miR-183, -137) exhibit distinct expression patterns in whole-DRG as well as in neuron-specific approaches. Therefore, further investigation of their possible role in initiation and maintenance of neuropathic pain seems promising.
Finally, the differential expression of genes like Corticoliberin or Otopetrin 1, previously not described in neuropathic pain, has already resulted in follow-up projects.
Peripheral neuropathy is accompanied by changes in the neuronal environment. The blood-nerve barrier (BNB) is crucial in protecting the neural homeostasis: Tight junctions (TJ) seal paracellular spaces and thus prevent external stimuli from entering. In different models of neuropathic pain, the BNB is impaired, thus contributing to local damage, immune cell invasion and, ultimately, the development of neuropathy with its symptoms. In this study, we examined changes in expression and microstructural localization of two key tight junction proteins (TJP), claudin-1 and the cytoplasmic anchoring ZO-1, in the sciatic nerve of mice subjected to chronic constriction injury (CCI). Via qPCR and analysis of fluorescence immunohistochemistry, a marked downregulation of mRNA as well as decreased fluorescence intensity were observed in the nerve for both proteins. Moreover, a distinct zig-zag structure for both proteins located at cell-cell contacts, indicative of the localization of TJs, was observed in the perineurial compartment of sham-operated animals. This microstructural location in cell-cell-contacts was lost in neuropathy as semiquantified via computational analysis, based on a novel algorithm. In summary, we provide evidence that peripheral neuropathy is not only associated with decrease in relevant TJPs but also exhibits alterations in TJP arrangement and loss in barrier tightness, presumably due to internalization. Specifically, semiquantification of TJP in cell-cell-contacts of microcompartments could be used in the future for routine clinical samples of patients with neuropathy.
Both nerve injury and complex regional pain syndrome (CRPS) can result in chronic pain. In traumatic neuropathy, the blood nerve barrier (BNB) shielding the nerve is impaired—partly due to dysregulated microRNAs (miRNAs). Upregulation of microRNA-21-5p (miR-21) has previously been documented in neuropathic pain, predominantly due to its proinflammatory features. However, little is known about other functions. Here, we characterized miR-21 in neuropathic pain and its impact on the BNB in a human-murine back translational approach. MiR-21 expression was elevated in plasma of patients with CRPS as well as in nerves of mice after transient and persistent nerve injury. Mice presented with BNB leakage, as well as loss of claudin-1 in both injured and spared nerves. Moreover, the putative miR-21 target RECK was decreased and downstream Mmp9 upregulated, as was Tgfb. In vitro experiments in human epithelial cells confirmed a downregulation of CLDN1 by miR-21 mimics via inhibition of the RECK/MMP9 pathway but not TGFB. Perineurial miR-21 mimic application in mice elicited mechanical hypersensitivity, while local inhibition of miR-21 after nerve injury reversed it. In summary, the data support a novel role for miR-21, independent of prior inflammation, in elicitation of pain and impairment of the BNB via RECK/MMP9.
Neuropathic pain, caused by a lesion in the somatosensory system, is a severely impairing mostly chronic disease. While its underlying molecular mechanisms are not thoroughly understood, neuroimmune interactions as well as changes in the pain pathway such as sensitization of nociceptors have been implicated. It has been shown that not only are different cell types involved in generation and maintenance of neuropathic pain, like neurons, immune and glial cells, but, also, intact adjacent neurons are relevant to the process. Here, we describe an experimental approach to discriminate damaged from intact adjacent neurons in the same dorsal root ganglion (DRG) using differential fluorescent neuronal labelling and fluorescence-activated cell sorting (FACS). Two fluorescent tracers, Fluoroemerald (FE) and 1-dioctadecyl-3,3,3,3-tetramethylindocarbocyanine perchlorate (DiI), were used, whose properties allow us to distinguish between damaged and intact neurons. Subsequent sorting permitted transcriptional analysis of both groups. Results and qPCR validation show a strong regulation in damaged neurons versus contralateral controls as well as a moderate regulation in adjacent neurons. Data for damaged neurons reveal an mRNA expression pattern consistent with established upregulated genes like galanin, which supports our approach. Moreover, novel genes were found strongly regulated such as corticotropinreleasing hormone (CRH), providing novel targets for further research. Differential fluorescent neuronal labelling and sorting allows for a clear distinction between primarily damaged neuropathic neurons and "bystanders," thereby facilitating a more detailed understanding of their respective roles in neuropathic processes in the DRG.
Das Mammakarzinom ist weltweit die häufigste krebsbedingte Todesursache bei Frauen. Fortschritte in der Therapie ermöglichen zwar eine Verlängerung der Lebens- dauer, jedoch kommt es dadurch vermehrt zur Bildung von Metastasen im zentralen Nervensystem (ZNS). Die Diagnostik und Behandlung von ZNS-Metastasen sind be- grenzt und die Lebensqualität sowie Lebensdauer der Betroffenen nimmt bei zerebraler Metastasierung rapide ab. Ziel aktueller Forschungsprojekte ist daher, Biomarker zu identifizieren, die Hinweise auf eine Brustkrebserkrankung oder Metastasierung liefern. So soll eine kostengünstige, risikoarme und minimalinvasive Methode etabliert werden, die zuverlässige Daten über die Prognose und dementsprechende Therapien erbringt. Diese Arbeit hatte daher die Absicht, mithilfe von qPCR Expressionsprofile von miRNAs aus Serumproben von Brustkrebspatientinnen zu erstellen und deren Funktion als prog- nostische Biomarker für eine Metastasierung ins ZNS zu erweisen. Anhand von Metas- tasierung und Rezeptorstatus wurden die Proben in Untergruppen eingeteilt und statis- tisch mit einer gesunden Kontrollgruppe verglichen.
Insgesamt zeigte sich bei 26 miRNAs eine signifikante Dysregulation der Expression bei mindestens einer der Untergruppen. Insbesondere bei ZNS-Metastasen war das Expres- sionsmuster bei miRNA-122-5p, miRNA-296-5p, miRNA-490-3p und miRNA-576-3p sig- nifikant erhöht, während die Expression von miRNA-130a-3p, miRNA-148b-3p und miRNA-326 signifikant reduziert war. Basierend auf den Übereinstimmungen unserer Er- gebnisse mit den Daten bisheriger Forschungsprojekten wiesen vier miRNAs eine po- tenzielle Funktion als Biomarker für Metastasen auf: miRNA-122-5p, miRNA-490-3p und miRNA-130a-3p, miRNA-326. Bei ZNS-Metastasen zeigten besonders miRNA-122-5p und miRNA-490-3p statistisch relevante Veränderungen.
Um den Einfluss von miRNAs auf den gesamten Körper darzustellen, wurde mithilfe ver- schiedener Datenbanken nach entsprechenden Zielgenen und Signalwegen für die 26 identifizierten miRNAs recherchiert. Neben dem Einfluss auf Stoffwechselwege und Er- krankungen, zeigte sich bei acht Targets ein Zusammenhang mit der Entstehung von Krebs.
Ergänzend zur Identifikation von miRNA-Expressionsprofilen wurden Zellkulturversuche mit zerebralen Endothel- (cerebEND) und Brustkrebszellen (4T1) durchgeführt. Verwendet wurden zwei cerebEND- und eine 4T1-Zellreihe von Mäusen, von denen eine ce- rebEND-Kultur zuvor in der Arbeitsgruppe Burek mit einem miRNA-210-Vektor trans- fiziert wurde.
Studien belegen den Einfluss von miRNA-210 auf den mitochondrialen Stoffwechsel, Angiogenese, Reaktionen auf DNA-Schäden, Apoptose und Zellüberleben sowie auf die Proteine BRCA1, PARP1 und E-Cadherin und schreiben ihr damit eine Funktion in der Krebsentstehung und Metastasierung zu.
Zur Bestimmung der Proliferation und Aktivität der transfizierten cerebEND-210-Zellen im Verhältnis zur unbehandelten Kontrolle, wurden BrdU-Proliferationsassays und MTT- Assays mit verschiedenen Zellzahlen durchgeführt. Bei der Untersuchung der Prolifera- tion zeigte sich in beiden Versuchen eine erhöhte Aktivität der cerebEND-210-Zellen, da miRNA-210 vermutlich auch hier das Zellüberleben gesichert hat. Zudem wurde die An- heftung der Brustkrebszellen an den zerebralen Endothelzellen im Adhäsionsversuchs überprüft. Hierbei wurde eine Abnahme der Adhäsion der cerebEND-210-Zellen beo- bachtet. Vermutet wird eine Veränderung des Phänotyps der Rezeptorbindungen der cerebEND-210-Zellen.
Die Ergebnisse der Zellkulturversuche dienen als Grundlage für weitere Experimente.
Background
Left atrial appendage (LAA) is the origin of most heart thrombi which can lead to stroke or other cerebrovascular event in patients with non-valvular atrial fibrillation (AF). This study aimed to prove safety and low complication rate of surgical LAA amputation using cut and sew technique with control of its effectiveness.
Methods
303 patients who have undergone selective LAA amputation were enrolled in the study in a period from 10/17 to 08/20. The LAA amputation was performed concomitant to routine cardiac surgery on cardiopulmonary bypass with cardiac arrest with or without previous history of AF. The operative and clinical data were evaluated. Extent of LAA amputation was examined intraoperatively by transoesophageal echocardiography (TEE). Six months in follow up, the patients were controlled regarding clinical status and episodes of strokes.
Results
Average age of study population was 69.9 ± 19.2 and 81.9% of patients were male. In only three patients was residual stump after LAA amputation larger than 1 cm with average stump size 0.28 ± 0.34 cm. 3 patients (1%) developed postoperative bleeding. Postoperatively 77 (25.4%) patients developed postoperative AF (POAF), of which 29 (9.6%) still had AF at discharge. On 6 months follow up only 5 patients had NYHA class III and 1 NYHA class IV. Seven patients reported with leg oedema and no patient experienced any cerebrovascular event in early postoperative follow up.
Conclusion
LAA amputation can be performed safely and completely leaving minimal to no LAA residual stump.
D-4F, ein ApoA-I-mimetisches Peptid, lindert mechanische Hyperalgesie nach Verabreichung an Nagetiere, die an entzündlichen und neuropathischen Schmerzen leiden. D-4F fängt proalgetische oxidierte Lipide ab – als ein solches entzündungshemmendes Medikament – aktiviert aber auch ATP-bindende Kassettentransporter (Abca1 und Abcg1) – als ein solches anti-atherosklerotisches Medikament. Ziel dieser Arbeit war es, den Einfluss von Neuropathie und deren Behandlung mit D-4F auf die Expression von Abca1/Abcg1 sowie Zytokinen zu untersuchen.
Im Rahmen des Qualitätsmanagements sollte eine Analyse der First-Responder- (FR) bzw. Helfer-vor-Ort (HvO) Einsätze in Würzburg und Umgebung nach dem zweiten Jahr nach Einführung auf der Basis der verpflichtenden Dokumentation erfolgen. Es sollte vor allem geprüft werden, ob der Einsatz der FR medizinisch nachweisbare Vorteile für die Notfallopfer bringt. Darüber hinaus sollte das Einsatzspektrum sowie der Umfang der erbrachten Leistungen analysiert werden. Grundsätzlich ist das möglichst schnelle Eintreffen der First-Responder am Einsatzort eine wesentliche Voraussetzung für den Erfolg der Notfallhilfe. Daher sollten in den verschiedenen Einsatzstellen die Abläufe von der Alarmierung bis zur Anfahrt nachträglich genau analysiert werden, um etwaige Schwachstellen aufzudecken, die möglicherweise zu einer verlängerten Anfahrtszeit führten. Darüber hinaus sollte hinterfragt werden, in wie weit Notfallzeugen bis zum eintreffen der FR bereits eine Erst-Hilfe geleistet haben. Letztendlich sollte analysiert werden wie groß die Differenzen zwischen präklinischen (Verdachts-) und postklinischen (Entlassungs-) Diagnosen wirklich sind.

Volumensubstitution ist ein elementarer Bestandteil bei der hämodynamischen Stabilisierung von Patienten bei großen operativen Eingriffen. Die Art und Menge der zu applizierenden Lösungen ist Gegenstand von kontrovers geführten Diskussionen. Ziel dieser Arbeit war es, die unterschiedlichen Auswirkungen der intraoperativ verabreichten kolloidalen Infusionslösungen 6% HES 130/0,4 (HES) und Humanalbumin 5% (HA) zu untersuchen.
Material & Methoden:
Retrospektive Datenanalyse (2000-2007) von neurochirurgisch versorgten Kindern <5 Jahren mit einem intraoperativen Blutverlust von > 10%, die entweder HES oder HA zur Volumensubstitution verabreicht bekommen hatten.
Ergebnisse:
913 Patienten wurden zwischen 2000-2007 operiert, wovon 30 mit HES (Alter 26,8±17,6 [Monate]; Gewicht 12,3±4,0 [kg]) und 56 mit HA behandelte Kinder (Alter 20,0±17,9; Gewicht 11,0±3,9) eingeschlossen werden konnten. Der Blutverlust in der HA-Gruppe (54,8±45,0 [% Blutverlust vom Gesamtblutvolumen]) war signifikant höher als in der HES-Gruppe (30,5±30,0). In der HA-Gruppe wurden signifikant mehr Kolloide (25,7±11,3 HA vs. 16,4±9,2 HES [ml/kgKG]) im Vergleich zu den mit HES behandelten Kindern verabreicht. Sowohl intraoperativ, als auch nach dem Intensivstation-Aufenthalt hatte die HA-Gruppe eine signifikant höhere positive Bilanz gegenüber der HES-Gruppe. Die Urinproduktion unterschied sich sowohl intra-, als auch postoperativ nicht zwischen den Gruppen. Bei den HA-Kindern war postoperativ die partielle Thromboplastinzeit (PTT) signifikant erhöht und das Fibrinogen erniedrigt. Interessanterweise zeigten aber die mit HA behandelten Kinder tendenziell einen kürzeren Intensivaufenthalt (40,2±25,1 HES vs. 31,6±24,3 HA [h]) und eine verkürzte postoperative Krankenhausverweildauer (HES 11,0±3,8; HA 9,8±4,6 [d]).
Schlussfolgerung:
Trotz größerem Blutverlust, höherem Bedarf an Kolloiden und erhöhter PTT zeigten die mit HA behandelten Kinder sowohl einen Trend zu einem kürzeren Intensivaufenthalt, als auch zu einer verkürzten postoperativen Krankenhausverweildauer. Für eine valide Aussage PRO HES oder HA zur Volumentherapie in der pädiatrischen Neurochirurgie, müsste eine doppelblinde randomisierte Studie durchgeführt werden.
Kardiale Ischämien sind die häufigste Todesursache in Deutschland. Aufgrund bisheri-ger Fortschritte in der Therapie und Entwicklung neuer präventiver Prozesse, gewinnen die Erkenntnisse kardioprotektiver Strategien zunehmend an Bedeutung und gelangen in den Mittelpunkt gegenwärtiger Forschung. Der Ablauf molekularer Vorgänge und die subzellulären Veränderungen im Rahmen der APost könnten zu einem möglichen Erklä-rungsansatz beitragen. Die APost wurde im Tiermodell bisher in diversen Spezies gezeigt und konnte experimentell etabliert werden. Diese Arbeit beschäftigt sich mit der Frage, ob die Veränderungen der GRK2 auf molekularer Ebene einer der Gründe für den kardioprotektiven Mechanismus der APost sein könnte. Die GRK2 ist eine myokardspezfische Kinase, die bereits im Rahmen von Herzinsuffizenzmodellen mit kardioprotektiven Eigenschaften in Zusammenhang gebracht worden ist. Ob die GRK2 ebenfalls im Ischämie/Reperfusionsmodell den letalen Reperfusionsschaden und die Infarktgöße beeinflusst, wird in dieser Arbeit in-vivo untersucht.
Zusammenfassend lassen sich aus dieser Studie folgende Erkenntnisse gewinnen:
Die Infarktgröße lässt sich durch Applikation von Sevofluran signifikant senken.
Die medikamentöse GRK2-Inhibition resultiert in einer signifikant verkleinerten In-farktgröße.

Diese Daten im Ischämie/Reperfusionsmodell zu reproduzieren und translational weiterzuentwickeln, sollte ein fokussiertes Ziel der Grundlagenforschung und Organprotektion werden, da unter Berücksichtigung des demographischen Wandels und der Lebenszeitprävalenzen zu KHK und Herzinfarkten ein gezieltes pharmakologisches Target im Fokus stehen sollte.
Schmerz ist eine klassische Komponente von Entzündungsreaktionen. Im Rahmen des Entzündungsgeschehens werden Zytokine und Chemokine freigesetzt, die Leukozyten zum Entzündungsort rekrutieren. Über die Freisetzung weiterer proalgetischer Mediatoren tragen diese zur Aktivierung und Sensitivierung von Nozizeptoren und damit zur Schmerzentstehung bei. Das Monozyten-rekrutierende Chemokin CCL2 verursachte in Verhaltensexperimenten eine Hyperalgesie bei Ratten. Die Hyperalgesie war durch den Cox-2 Inhibitor Parecoxib vollständig reversibel. Daher wurde in dieser Arbeit die Rolle von Monozyten und Prostaglandinen in der Entstehung dieser Hyperalgesie untersucht. Dazu wurde in vitro die Cox-2 Expression und die Prostaglandin-Bildung in humanen Monozyten und Peritonealmakrophagen der Ratte nach CCL2 Stimulation bestimmt. Zudem wurde in vivo die Cox-2 Expression im Rückenmark und in der Rattenpfote nach CCL2 Injektion in die Pfote untersucht.
Die wichtigsten Barrieren des peripheren Nerven sind die Myelin-Barriere und die Blut-Nerven-Schranke. Sie übernehmen eine bedeutende Aufgabe beim Schutz des Nerven vor externen Einflüssen, dem Bewahren der nervalen Homöostase und der Aufrechterhaltung der Nervenleitungsgeschwindigkeit. Eine Öffnung der peripheren Barriere wird oft mit neuropathischen Schmerzen assoziiert. Im Rahmen dieser Dissertation habe ich mittels Immunfluoreszenzfärbung, Permeabilitätstest, Western Blot und Polymerase-Kettenreaktion die Barrierefunktion des peripheren Nervensystems bei weiblichen Cldn12-KO Mäusen untersucht. Sowohl bei den WT Mäusen als auch bei den Cldn12-KO Mäusen konnte eine intakte Barrierefunktion des peripheren Nerven nachgewiesen werden.
Postoperative pulmonale Komplikationen (PPK) gehören zu den häufigsten Ursachen, die zu einem verlängerten Krankenhausaufenthalt, steigenden Kosten und einer erhöhten Morbidität und Mortalität führen. Die pulmonale elektrische Impedanztomographie (EIT) ermöglicht die bettseitige Visualisierung der regionalen Lungenventilation in Echtzeit in der transversalen Elektrodenebene. Insbesondere dorsale Atelektasen oder Pleuraergüsse können eine Verschiebung der Ventilation nach ventral bewirken.
Als Hypothese wurde angenommen, dass postoperativ bei spontanatmenden Patienten nach einem peripheren Eingriff eine Umverteilung der Ventilation nach ventral auftritt und dadurch die Vitalkapazität der Lunge abnimmt.
In die prospektive Beobachtungstudie wurden 27 Patienten eingeschlossen, die sich einem elektiven peripheren Eingriff in Allgemeinnarkose (mit oder ohne Regionalanästhesie) unterzogen und ein erhöhtes Risiko für PPK‘s aufwiesen. Die Lungenfunktion der Patienten wurde präoperativ sowie am ersten und dritten postoperativen Tag mittels EIT in Spontanatmung und einem forcierten Atemmanöver erfasst. Dabei wurde das Center of Ventilation (COVy) berechnet.
Periphere Eingriffe in Allgemeinnarkose führten nicht zu einer signifikanten ventralen Verschiebung der regionalen pulmonalen Ventilation in Spontanatmung gemäß pulmonalem EIT am ersten und dritten postoperativen Tag. Die FVC%Soll war lediglich am ersten postoperativen Tag vermindert. Bei zwei Patienten traten postoperative pulmonale Komplikationen auf. Insgesamt zeigte die Kohorte nur geringe Veränderungen der Vitalkapazität, was einen Einfluss der Eingriffslokalisation auf die Umverteilung der pulmonalen Ventilation nahelegt.
Antinociceptive pathways are activated in the periphery in inflammatory pain, for instance resolvins and opioid peptides. Resolvins are biosynthesized from omega-3 polyunsaturated fatty acids such as eicosapentaenoic acid and docosahexaenoic acid. Resolvin D1 (RvD1) and resolvin E1 (RvE1) initiate the resolution of inflammation and control of hypersensitivity via induction of anti-inflammatory signaling cascades. RvD1 binds to lipoxin A4/annexin-A1 receptor/formyl-peptide receptor 2 (ALX/FPR2), RvE1 to chemerin receptor 23 (ChemR23). Antinociception of RvD1 is mediated by interaction with transient receptor potential channels ankyrin 1 (TRPA1). Endogenous opioid peptides are synthesized and released from leukocytes in the tissue and bind to opioid receptors on nociceptor terminals. Here, we further explored peripheral mechanisms of RvD1 and chemerin (Chem), the ligand of ChemR23, in complete Freund’s adjuvant (CFA)-induced hindpaw inflammation in male Wistar rats. RvD1 and Chem ameliorated CFA-induced hypersensitivity in early and late inflammatory phases. This was prevented by peripheral blockade of the μ-opioid peptide receptor (MOR) using low dose local naloxone or by local injection of anti-β-endorphin and anti-met-enkephalin (anti-ENK) antibodies. Naloxone also hindered antinociception by the TRPA1 inhibitor HC-030031. RvD1 did not stimulate the release of β-endorphin from macrophages and neutrophils, nor did RvD1 itself activate G-proteins coupled MOR or initiate β-arrestin recruitment to the membrane. TRPA1 blockade by HC-030031 in inflammation in vivo as well as inhibition of the TRPA1-mediated calcium influx in dorsal root ganglia neurons in vitro was hampered by naloxone. Peripheral application of naloxone alone in vivo already lowered mechanical nociceptive thresholds. Therefore, either a perturbation of the balance of endogenous pro- and antinociceptive mechanisms in early and late inflammation, or an interaction of TRPA1 and opioid receptors weaken the antinociceptive potency of RvD1 and TRPA1 blockers.
Background
High doses of capsaicin are recommended for the treatment of neuropathic pain. However, low doses evoke mechanical hypersensitivity. Activation of the capsaicin chemosensor transient receptor potential vanilloid 1 (TRPV1) induces neurogenic inflammation. In addition to the release of pro-inflammatory mediators, reactive oxygen species are produced. These highly reactive molecules generate oxidised phospholipids and 4-hydroxynonenal (4-HNE) which then directly activate TRP ankyrin 1 (TRPA1). The apolipoprotein A-I mimetic peptide D-4F neutralises oxidised phospholipids. Here, we asked whether D-4F ameliorates neurogenic hypersensitivity in rodents by targeting reactive oxygen species and 4-HNE in the capsaicin-evoked pain model.
Results
Co-application of D-4F ameliorated capsaicin-induced mechanical hypersensitivity and allodynia as well as persistent heat hypersensitivity measured by Randell–Selitto, von Frey and Hargreaves test, respectively. In addition, mechanical hypersensitivity was blocked after co-injection of D-4F with the reactive oxygen species analogue H2O2 or 4-HNE. In vitro studies on dorsal root ganglion neurons and stably transfected cell lines revealed a TRPA1-dependent inhibition of the calcium influx when agonists were pre-incubated with D-4F. The capsaicin-induced calcium influx in TRPV1-expressing cell lines and dorsal root ganglion neurons sustained in the presence of D-4F.
Conclusions
D-4F is a promising compound to ameliorate TRPA1-dependent hypersensitivity during neurogenic inflammation.
Phospholipids occurring in cell membranes and lipoproteins are converted into oxidized phospholipids (OxPL) by oxidative stress promoting atherosclerotic plaque formation. Here, OxPL were characterized as novel targets in acute and chronic inflammatory pain. Oxidized 1-palmitoyl-2-arachidonoyl-sn-glycero-3-phosphocholine (OxPAPC) and its derivatives were identified in inflamed tissue by mass spectrometry and binding assays. They elicited calcium influx, hyperalgesia and induced pro-nociceptive peptide release. Genetic, pharmacological and mass spectrometric evidence in vivo as well as in vitro confirmed the role of transient receptor potential channels (TRPA1 and TRPV1) as OxPAPC targets. Treatment with the monoclonal antibody E06 or with apolipoprotein A-I mimetic peptide D-4F, capturing OxPAPC in atherosclerosis, prevented inflammatory hyperalgesia, and in vitro TRPA1 activation. Administration of D-4F or E06 to rats profoundly ameliorated mechanical hyperalgesia and inflammation in collagen-induced arthritis. These data reveal a clinically relevant role for OxPAPC in inflammation offering therapy for acute and chronic inflammatory pain treatment by scavenging OxPAPC.
Within the lipidome oxidized phospholipids (OxPL) form a class of chemically highly reactive metabolites. OxPL are acutely produced in inflamed tissue and act as endogenous, proalgesic (pain-inducing) metabolites. They excite sensory, nociceptive neurons by activating transient receptor potential ion channels, specifically TRPA1 and TRPV1. Under inflammatory conditions, OxPL-mediated receptor potentials even potentiate the action potential firing rate of nociceptors. Targeting OxPL with D-4F, an apolipoprotein A-I mimetic peptide or antibodies like E06, specifically binding oxidized headgroups of phospholipids, can be used to control acute, inflammatory pain syndromes, at least in rodents. With a focus on proalgesic specificities of OxPL, this article discusses, how targeting defined substances of the epilipidome can contribute to mechanism-based therapies against primary and secondary chronic inflammatory or possibly also neuropathic pain.
Chronic stress, even stress of a moderate intensity related to daily life, is widely acknowledged to be a predisposing or precipitating factor in neuropsychiatric diseases. There is a clear relationship between disturbances induced by stressful stimuli, especially long-lasting stimuli, and cognitive deficits in rodent models of affective disorders. Regular physical activity has a positive effect on the central nervous system (CNS) functions, contributes to an improvement in mood and of cognitive abilities (including memory and learning), and is correlated with an increase in the expression of the neurotrophic factors and markers of synaptic plasticity as well as a reduction in the inflammatory factors. Studies published so far show that the energy challenge caused by physical exercise can affect the CNS by improving cellular bioenergetics, stimulating the processes responsible for the removal of damaged organelles and molecules, and attenuating inflammation processes. Regular physical activity brings another important benefit: increased stress robustness. The evidence from animal studies is that a sedentary lifestyle is associated with stress vulnerability, whereas a physically active lifestyle is associated with stress resilience. Here, we have performed a comprehensive PubMed Search Strategy for accomplishing an exhaustive literature review. In this review, we discuss the findings from experimental studies on the molecular and neurobiological mechanisms underlying the impact of exercise on brain resilience. A thorough understanding of the mechanisms underlying the neuroprotective potential of preconditioning exercise and of the role of exercise in stress resilience, among other things, may open further options for prevention and therapy in the treatment of CNS diseases.
The aim of this work was to conduct a comprehensive study about the transport properties of NSAIDs across the blood-brain barrier (BBB) in vitro. Transport studies with celecoxib, diclofenac, ibuprofen, meloxicam, piroxicam and tenoxicam were accomplished across Transwell models based on cell line PBMEC/C1-2, ECV304 or primary rat brain endothelial cells. Single as well as group substance studies were carried out. In group studies substance group compositions, transport medium and serum content were varied, transport inhibitors verapamil and probenecid were added. Resulted permeability coefficients were compared and normalized to internal standards diazepam and carboxyfluorescein. Transport rankings of NSAIDs across each model were obtained. Single substance studies showed similar rankings as corresponding group studies across PBMEC/C1-2 or ECV304 cell layers. Serum content, glioma conditioned medium and inhibitors probenecid and verapamil influenced resulted permeability significantly. Basic differences of transport properties of the investigated NSAIDs were similar comparing all three in vitro BBB models. Different substance combinations in the group studies and addition of probenecid and verapamil suggested that transporter proteins are involved in the transport of every tested NSAID. Results especially underlined the importance of same experimental conditions (transport medium, serum content, species origin, cell line) for proper data comparison.
Objectives
The severity of Coronavirus Disease 2019 (COVID-19) is largely determined by the immune response. First studies indicate altered lymphocyte counts and function. However, interactions of pro- and anti-inflammatory mechanisms remain elusive. In the current study we characterized the immune responses in patients suffering from severe COVID-19-induced acute respiratory distress syndrome (ARDS).
Methods
This was a single-center retrospective study in patients admitted to the intensive care unit (ICU) with confirmed COVID-19 between March 14th and May 28th 2020 (n = 39). Longitudinal data were collected within routine clinical care, including flow-cytometry of lymphocyte subsets, cytokine analysis and growth differentiation factor 15 (GDF-15). Antibody responses against the receptor binding domain (RBD) of Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) Spike protein were analyzed.
Results
All patients suffered from severe ARDS, 30.8% died. Interleukin (IL)-6 was massively elevated at every time-point. The anti-inflammatory cytokine IL-10 was concomitantly upregulated with IL-6. The cellular response was characterized by lymphocytopenia with low counts of CD8+ T cells, natural killer (NK) and naïve T helper cells. CD8+ T and NK cells recovered after 8 to 14 days. The B cell system was largely unimpeded. This coincided with a slight increase in anti-SARS-CoV-2-Spike-RBD immunoglobulin (Ig) G and a decrease in anti-SARS-CoV-2-Spike-RBD IgM. GDF-15 levels were elevated throughout ICU treatment.
Conclusions
Massively elevated levels of IL-6 and a delayed cytotoxic immune defense characterized severe COVID-19-induced ARDS. The B cell response and antibody production were largely unimpeded. No obvious imbalance of pro- and anti-inflammatory mechanisms was observed, with elevated GDF-15 levels suggesting increased tissue resilience.
Background
Parenteral lipid emulsions in critical care are traditionally based on soybean oil (SO) and rich in pro-inflammatory omega-6 fatty acids (FAs). Parenteral nutrition (PN) strategies with the aim of reducing omega-6 FAs may potentially decrease the morbidity and mortality in critically ill patients.
Methods
A systematic search of MEDLINE, EMBASE, CINAHL and CENTRAL was conducted to identify all randomized controlled trials in critically ill patients published from inception to June 2021, which investigated clinical omega-6 sparing effects. Two independent reviewers extracted bias risk, treatment details, patient characteristics and clinical outcomes. Random effect meta-analysis was performed.
Results
1054 studies were identified in our electronic search, 136 trials were assessed for eligibility and 26 trials with 1733 critically ill patients were included. The median methodologic score was 9 out of 14 points (95% confidence interval [CI] 7, 10). Omega-6 FA sparing PN in comparison with traditional lipid emulsions did not decrease overall mortality (20 studies; risk ratio [RR] 0.91; 95% CI 0.76, 1.10; p = 0.34) but hospital length of stay was substantially reduced (6 studies; weighted mean difference [WMD] − 6.88; 95% CI − 11.27, − 2.49; p = 0.002). Among the different lipid emulsions, fish oil (FO) containing PN reduced the length of intensive care (8 studies; WMD − 3.53; 95% CI − 6.16, − 0.90; p = 0.009) and rate of infectious complications (4 studies; RR 0.65; 95% CI 0.44, 0.95; p = 0.03). When FO was administered as a stand-alone medication outside PN, potential mortality benefits were observed compared to standard care.
Conclusion
Overall, these findings highlight distinctive omega-6 sparing effects attributed to PN. Among the different lipid emulsions, FO in combination with PN or as a stand-alone treatment may have the greatest clinical impact.
Background
Recent data from the randomized SUSTAIN CSX trial could not confirm clinical benefits from perioperative selenium treatment in high-risk cardiac surgery patients. Underlying reasons may involve inadequate biosynthesis of glutathione peroxidase (GPx3), which is a key mediator of selenium's antioxidant effects. This secondary analysis aimed to identify patients with an increase in GPx3 activity following selenium treatment. We hypothesize that these responders might benefit from perioperative selenium treatment.
Methods
Patients were selected based on the availability of selenium biomarker information. Four subgroups were defined according to the patient's baseline status, including those with normal kidney function, reduced kidney function, selenium deficiency, and submaximal GPx3 activity.
Results
Two hundred and forty-four patients were included in this analysis. Overall, higher serum concentrations of selenium, selenoprotein P (SELENOP) and GPx3 were correlated with less organ injury. GPx3 activity at baseline was predictive of 6-month survival (AUC 0.73; p = 0.03). While selenium treatment elevated serum selenium and SELENOP concentrations but not GPx3 activity in the full patient cohort, subgroup analyses revealed that GPx3 activity increased in patients with reduced kidney function, selenium deficiency and low to moderate GPx3 activity. Clinical outcomes did not vary between selenium treatment and placebo in any of these subgroups, though the study was not powered to conclusively detect differences in outcomes.
Conclusions
The identification of GPx3 responders encourages further refined investigations into the treatment effects of selenium in high-risk cardiac surgery patients.
The interplay between inflammation and oxidative stress is a vicious circle, potentially resulting in organ damage. Essential micronutrients such as selenium (Se) and zinc (Zn) support anti-oxidative defense systems and are commonly depleted in severe disease. This single-center retrospective study investigated micronutrient levels under Se and Zn supplementation in critically ill patients with COVID-19 induced acute respiratory distress syndrome (ARDS) and explored potential relationships with immunological and clinical parameters. According to intensive care unit (ICU) standard operating procedures, patients received 1.0 mg of intravenous Se daily on top of artificial nutrition, which contained various amounts of Se and Zn. Micronutrients, inflammatory cytokines, lymphocyte subsets and clinical data were extracted from the patient data management system on admission and after 10 to 14 days of treatment. Forty-six patients were screened for eligibility and 22 patients were included in the study. Twenty-one patients (95%) suffered from severe ARDS and 14 patients (64%) survived to ICU discharge. On admission, the majority of patients had low Se status biomarkers and Zn levels, along with elevated inflammatory parameters. Se supplementation significantly elevated Se (p = 0.027) and selenoprotein P levels (SELENOP; p = 0.016) to normal range. Accordingly, glutathione peroxidase 3 (GPx3) activity increased over time (p = 0.021). Se biomarkers, most notably SELENOP, were inversely correlated with CRP (r\(_s\) = −0.495), PCT (r\(_s\) = −0.413), IL-6 (r\(_s\) = −0.429), IL-1β (r\(_s\) = −0.440) and IL-10 (r\(_s\) = −0.461). Positive associations were found for CD8\(^+\) T cells (r(_s\) = 0.636), NK cells (r\(_s\) = 0.772), total IgG (r\(_s\) = 0.493) and PaO\(_2\)/FiO\(_2\) ratios (r\(_s\) = 0.504). In addition, survivors tended to have higher Se levels after 10 to 14 days compared to non-survivors (p = 0.075). Sufficient Se and Zn levels may potentially be of clinical significance for an adequate immune response in critically ill patients with severe COVID-19 ARDS.
Diabetische Polyneuropathie ist die häufigste Folgeerkrankung eines Diabetes mellitus. Bei ca. 20 % der betroffenen Patienten tritt eine schmerzhafte Form der Polyneuropathie auf.
Eine intakte Blut-Nerven-Barriere hält im Endoneurium ein spezifisches Milieu aufrecht. Die Dichtigkeit der Blut-Nerven-Barriere (BNB) wird durch Tight Junctions im Perineurium und in endoneuralen Kapillaren hergestellt. Eine Öffnung der BNB in Kombination mit einem algetischen Stimulus ist ein wesentlicher Mechanismus neuropathischer Schmerzen in traumatischen Tiermodellen. Über den Stellenwert von Störungen der BNB bei diabetischer Polyneuropathie wird kontrovers diskutiert. Diese Arbeit beleuchtet funktionelle Änderungen der BNB und die Expression wichtiger Tight-Junction-Proteine in einem Modell für schmerzhafte diabetische Polyneuropathie.
Nach Genehmigung durch die Regierung von Unterfranken und unter Einhaltung der ARRIVE-Richtlinien wurde eine experimentelle diabetische Polyneuropathie in Wistar-Ratten durch einmalige intravenöse Gabe von Streptozocin (STZ) induziert. Zwei Wochen nach Diabetesinduktion trat eine mechanische Allodynie auf. Nach acht Wochen war eine selektive Öffnung der BNB für die niedermolekulare Verbindung Fluorescein-Natrium (376 Da) in vivo und ex vivo nachweisbar. Für die makromolekulare Testsubstanz Evans blue Albumin (69 kDa) erwies sich die BNB als intakt. Eine verstärkte endoneurale Ansammlung von Makrophagen wurde immunhistochemisch nicht beobachtet. Die Expression wichtiger Tight-Junction-Proteine in ganzem peripherem Nerv, in Spinalganglien und im Rückenmark wies keine signifikanten Änderungen in der quantitativen Echtzeit-PCR auf. Eine selektive Analyse nach Lasermikrodissektion zeigte jedoch eine Minderexpression von Cldn5 in endoneuralen Gefäßen und Cldn1 im Perineurium nach acht Wochen.
Bei STZ-induzierter Polyneuropathie tritt somit eine größenselektive Öffnung der BNB auf, die sich zeitlich deutlich nach dem Beginn mechanischer Allodynie manifestiert. Die Öffnung korreliert mit einer Minderexpression von Cldn1 mRNA perineural und von Cldn5 mRNA in endoneuralen Gefäßen. In der multifaktoriellen Pathophysiologie der diabetischen Polyneuropathie kann die Öffnung der BNB als weiterer schädigender Kofaktor betrachtet werden, der zur Aufrechterhaltung neuropathischer Schmerzen beiträgt.
Die Blut-Nerven-Schranke stellt eine Barriere zwischen den Axonen und dem Gewebe, das sie umgibt, dar. Durch verschiedene Mechanismen wird die intraneurale Homöostase aufrecht erhalten, welche für die Funktion der Axone essentiell ist. Auf der einen Seite stellt die Blut-Nerven-Schranke einen Schutzmechanismus für Nerven dar. Auf der anderen Seite erschwert sie die Applikationen von Pharmaka, z.B. Analgetika, sodass bis dato Lokalanästhetika die einzige Substanzklasse darstellen, welche zur gezielten örtlichen, analgetischen Therapie in der breiten Verwendung ist. Das Netzwerk der Tight junctions im Perineurium ist eine Schlüsselkomponente für die Funktion der Blut-Nerven-Schranke. Es konnte experimentell gezeigt werden, dass durch pharmakologische Interaktion mit den Tight Junctions eine Schrankenöffnung erzielt werden kann. Dieser Effekt wurde durch unsere Arbeitsgruppe nachgewiesen für das von der N-terminalen Schleife des Claudin-1 abgeleitete Peptid C1C2, hypertone Substanzen, wie HTS und den Gewebeplasminogenaktivator tPA. Durch Anwendung solcher Wirkstoffvermittler könnte das Axon für andere Substanzklassen erreichbar werden. Die Fragestellung dieser Arbeit war, ob sich durch die lokale Anwendung dieser Pharmaka am N. ischiadicus von Wistar Ratten toxische Effekte ergeben. Hierzu wurden immunhistochemische Färbungen mit einem Anti-CD68- Antikörper zur Darstellung von endoneuralen Makrophagen als Zeichen einer lokalen Entzündungsreaktion verwendet. Weiterhin wurden die Nerven mit Hilfe histomorphometrischer Analysen auf morphologische Veränderungen hin untersucht. Keine der untersuchten Substanzen bewirkte eine Zunahme von endoneuralen Makrophagen im Vergleich zur scheinbehandelten Kontrollgruppe. Die Positivkontrolle eines N. ischiadicus nach chronischer Konstriktionsläsion (CCI) zeigte hingegen eine massive Vermehrung endoneuraler Makrophagen. Bei der morphometrischen Analyse zeigte sich nur bei der mit tPA behandelten Gruppe eine Abnahme der g-Ratio, welche das Verhältnis von Myelinschicht zu Faserdurchmesser wiederspiegelt. Die anderen Teilaspekte der Untersuchung blieben unauffällig. Aus dieser Arbeit kann geschlossen werden, dass sich mit den verwendeten Untersuchungsmethoden für keine der untersuchten Substanzen eine lokal toxische Wirkung nachweisen lässt. Um die Aussagekraft bezüglich der Anwendungssicherheit weiter zu erhöhen, wären im weiteren Verlauf Versuchsreihen mit Blick auf mögliche Langzeitwirkungen und etwaige systemtoxische Wirkungen von besonderer Bedeutung
Der Myokardinfarkt zählt weltweit zu den Hauptursachen für Tod und Erwerbsunfähigkeit. Große Bedeutung kommt der Untersuchung kardioprotektiver Mechanismen und der Etablierung kardioprotektiver Strategien zu. Die Anästhetika-induzierte Präkonditionierung weist einen biphasichen Verlauf auf und lässt sich in eine frühe und eine späte Phase (Zweites Fenster) einteilen. In der vorliegenden Arbeit wurde die Rolle der Pim-1 Kinase im Zweiten Fenster der Desfluran-induzierten Präkonditionierung gegen Myokardinfarkt in einem murinen in-vivo Herzinfarktmodell untersucht.

Antenatal steroid treatment decreases morbidity and mortality in premature infants through the maturation of lung tissue, which enables sufficient breathing performance. However, clinical and animal studies have shown that repeated doses of glucocorticoids such as dexamethasone and betamethasone lead to long-term adverse effects on brain development. Therefore, we established a mouse model for antenatal dexamethasone treatment to investigate the effects of dexamethasone on brain vessel differentiation towards the blood-brain barrier (BBB) phenotype, focusing on molecular marker analysis. The major findings were that in total brains on postnatal day (PN) 4 triple antenatal dexamethasone treatment significantly downregulated the tight junction protein claudin-5, the endothelial marker Pecam-1/CD31, the glucocorticoid receptor, the NR1 subunit of the N-methyl-D-aspartate receptor, and Abc transporters (Abcb1a, Abcg2 Abcc4). Less pronounced effects were found after single antenatal dexamethasone treatment and in PN10 samples. Comparisons of total brain samples with isolated brain endothelial cells together with the stainings for Pecam-1/CD31 and claudin-5 led to the assumption that the morphology of brain vessels is affected by antenatal dexamethasone treatment at PN4. On the mRNA level markers for angiogenesis, the sonic hedgehog and the Wnt pathway were downregulated in PN4 samples, suggesting fundamental changes in brain vascularization and/or differentiation. In conclusion, we provided a first comprehensive molecular basis for the adverse effects of multiple antenatal dexamethasone treatment on brain vessel differentiation.

The blood-air barrier in the lung consists of the alveolar epithelium, the underlying capillary endothelium, their basement membranes and the interstitial space between the cell layers. Little is known about the interactions between the alveolar and the blood compartment. The aim of the present study was to gain first insights into the possible interplay between these two neighboured cell layers. We established an in vitro Transwell model of the alveolar epithelium based on human cell line H441 and investigated the influence of conditioned medium obtained from human lung endothelial cell line HPMEC-ST1.6R on the barrier properties of the H441 layers. As control for tissue specificity H441 layers were exposed to conditioned medium from human brain endothelial cell line hCMEC/D3. Addition of dexamethasone was necessary to obtain stable H441 cell layers. Moreover, dexamethasone increased expression of cell type I markers (caveolin-1, RAGE) and cell type II marker SP-B, whereas decreased the transepithelial electrical resistance (TEER) in a concentration dependent manner. Soluble factors obtained from the lung endothelial cell line increased the barrier significantly proven by TEER values and fluorescein permeability on the functional level and by the differential expression of tight junctional proteins on the molecular level. In contrast to this, soluble factors derived from brain endothelial cells weakened the barrier significantly. In conclusion, soluble factors from lung endothelial cells can strengthen the alveolar epithelium barrier in vitro, which suggests communication between endothelial and epithelial cells regulating the integrity of the blood-air barrier.
Stabilization of the blood-brain barrier during and after stroke can lead to less adverse outcome. For elucidation of underlying mechanisms and development of novel therapeutic strategies validated in vitro disease models of the blood-brain barrier could be very helpful. To mimic in vitro stroke conditions we have established a blood-brain barrier in vitro model based on mouse cell line cerebEND and applied oxygen/glucose deprivation (OGD). The role of astrocytes in this disease model was investigated by using cell line C6. Transwell studies pointed out that addition of astrocytes during OGD increased the barrier damage significantly in comparison to the endothelial monoculture shown by changes of transendothelial electrical resistance as well as fluorescein permeability data. Analysis on mRNA and protein levels by qPCR, western blotting and immunofluorescence microscopy of tight junction molecules claudin-3,-5,-12, occludin and ZO-1 revealed that their regulation and localisation is associated with the functional barrier breakdown. Furthermore, soluble factors of astrocytes, OGD and their combination were able to induce changes of functionality and expression of ABC-transporters Abcb1a (P-gp), Abcg2 (bcrp), and Abcc4 (mrp4). Moreover, the expression of proteases (matrixmetalloproteinases MMP-2, MMP-3, MMP-9, and t-PA) as well as of their endogenous inhibitors (TIMP-1, TIMP-3, PAI-1) was altered by astrocyte factors and OGD which resulted in significant changes of total MMP and t-PA activity. Morphological rearrangements induced by OGD and treatment with astrocyte factors were confirmed at a nanometer scale using atomic force microscopy. In conclusion, astrocytes play a major role in blood-brain barrier breakdown during OGD in vitro.
During stroke the blood–brain barrier (BBB) is damaged which can result in vasogenic brain edema and inflammation. The reduced blood supply leads to decreased delivery of oxygen and glucose to affected areas of the brain. Oxygen and glucose deprivation (OGD) can cause upregulation of glucose uptake of brain endothelial cells. In this letter, we investigated the influence of MK801, a non-competitive inhibitor of the NMDA-receptor, on the regulation of the glucose uptake and of the main glucose transporters glut1 and sglt1 in murine BBB cell line cerebEND during OGD. mRNA expression of glut1 was upregulated 68.7- fold after 6 h OGD, which was significantly reduced by 10 μM MK801 to 28.9-fold. Sglt1 mRNA expression decreased during OGD which was further reduced by MK801. Glucose uptake was significantly increased up to 907% after 6 h OGD and was still higher (210%) after the 20 h reoxygenation phase compared to normoxia. Ten micromolar MK801 during OGD was able to reduce upregulated glucose uptake after OGD and reoxygenation significantly. Presence of several NMDAR subunits was proven on the mRNA level in cerebEND cells. Furthermore, it was shown that NMDAR subunit NR1 was upregulated during OGD and that this was inhibitable by MK801. In conclusion, the addition of MK801 during the OGD phase reduced significantly the glucose uptake after the subsequent reoxygenation phase in brain endothelial cells.
In einer systematischen Übersichtsarbeit mit Metaanalyse wurde chronischer Wundschmerz nach Sectio caesarea mit einer Inzidenz von jeweils 15,4%, 11,5% und 11,2% der Frauen nach 3 bis < 6, 6 bis < 12 und über 12 Monaten geschätzt. Chronische postoperative Schmerzen nach Kaiserschnitten sind somit ein relevantes klinisches Problem. Im zweiten Teil dieser Dissertation wurde in einer prospektiven Beobachtungsstudie die Inzidenz chronischer postoperativer Schmerzen nach 12 Monaten mit jeweils 12,1% nach Sectio caesarea, 13,7% nach Hysterektomie und 38,1% nach mammachirurgischen Eingriffen ermittelt. Als Risikofaktoren konnten die Komorbidität, der zeitliche Anteil akuter postoperativer starker Schmerzen sowie die Schmerzstärke der akuten postoperativen Schmerzen signifikant erkannt werden.
Überwachung der Narkosetiefe: Wissenschaft und Wirklichkeit Fragestellung: Die vorliegende systematische Übersicht soll einen Überblick über die wissenschaftlich getesteten Verfahren zur Bestimmung der Narkosetiefe schaffen. Darüber hinaus könnte die dadurch entstandene Wissensbank zur Errechnung eines kombinierten Narkosetiefenparameters dienen. Parallel dazu wird durch eine Umfrage die klinische Wirklichkeit abgebildet. Methoden: Mit Hilfe der Suchfunktionen der Internetdatenbanken der National Library of Medicine, Pubmed, Embase und Central aus der Cochrane Library wurden alle Veröffentlichungen über passende wissenschaftliche Studien zum Thema „Narkosetiefe-Monitoring“ ausgewählt, gesichtet, katalogisiert und verglichen. Gleichzeitig wurde eine Umfrage nach den tatsächlich angewandten Überwachungsverfahren der Narkosetiefe an 30 Universitätskliniken Deutschlands durchgeführt. Ergebnisse: Die Auswertung der 179 aus den Datenbanken ausgewählten Originalarbeiten ergab eine große Vielfalt an unterschiedlichen Möglichkeiten im Studiendesign. Es konnten vier gänzlich verschiedene Herangehensweisen definiert werden. Selbst innerhalb dieser vier Gruppen sind die Differenzen so groß, dass die jeweils zugeordneten Studien kaum untereinander verglichen werden können. Die Unterschiede wurden herausgestellt und tabellarisiert, so dass Studien mit ähnlichen oder gleichen Ansätzen, Bedingungen und Parametern erkannt werden können. Insgesamt fällt auf, dass in 70,8% der Studien EEG-basierte Monitore (wie z.B. BIS) mehr als in einem Vergleich oder als einziges untersucht wurden. Herz-Kreislauf-Parameter (wie z.B. Blutdruck und Herzfrequenz) wurden in 14,6% der Studien in mehr als einem Vergleich oder ausschließlich untersucht. Im Gegensatz dazu werden in den 313 Abteilungen der 30 befragten Universitätskliniken Deutschlands zu 9,3% EEG-basierte Monitore tatsächlich standardmäßig angewandt und zu 92,7 % Herz-Kreislauf-Parameter. Diskussion: Die Unterschiede in den 179 Originalarbeiten sind so zahlreich und vielfältig, dass aus ihren Ergebnissen keine Wissensbank für einen kombinierten Narkosetiefe-Parameter erstellt werden kann. In den Studien wurden hauptsächlich EEG-basierte Monitore untersucht, die im Gegensatz dazu in der klinischen Wirklichkeit nur selten und in bestimmten Situationen angewandt werden. Zu den am häufigsten eingesetzten Herz-Kreislauf-Parametern gibt es dagegen kaum geeignete und wenn nur schlecht vergleichbare wissenschaftliche Untersuchungen. Ein akzeptierter Goldstandart, der auch die für Patienten so schwerwiegenden Wachheitserlebnisse zuverlässig verhindern lässt, fehlt weiterhin. Schlussfolgerung: In Zukunft sollten Studien mit einheitlicheren Designs durchgeführt werden, damit direkte Vergleiche und Zusammenfassungen ermöglicht werden können.
Die Maligne Hyperthermie (MH) ist eine erbliche Myopathie, die bei prädisponierten Patienten durch Anwendung volatiler Anästhetika sowie depolarisierender Muskelrelaxanzien bei Narkosen verursacht wird. Die klinische Symptomatik umfasst einen Anstieg des endexspiratorischen Kohlendioxidpartialdrucks, metabolische Azidose und Hyperthermie. Dieser Prozess wird durch einen abnorm erhöhten Einstrom von Kalzium aus dem sarkoplasmatischen Retikulum in das Zytoplasma ausgelöst, welcher durch eine Mutation im Ryanodinrezeptor der Skelettmuskulatur bedingt ist. Der Goldstandard für die Diagnostik einer MH-Veranlagung ist der Koffein-Halothan-In-vitro-Kontrakturtest. Eine Alternative zu diesem operativen Eingriff stellt der minimal-invasive Test dar. Untersucht wurde, ob der intramuskuläre Laktatspiegel durch eine lokale Applikation der Triggersubstanzen Halothan 6 Vol%, Koffein 80 mM und Halothan 4 Vol% in Abhängigkeit von der MH-Prädisposition gesteigert wird. Ziel der Studie war es zu überprüfen, ob dies eine Differenzierung zwischen MHS- und MHN-Individuen ermöglicht. Da Halothan in naher Zukunft kommerziell nicht mehr erhältlich sein könnte, wurde Sevofluran als neueres Inhalationsanästhetikum im Tierversuch in unterschiedlichen Konzentrationen intramuskulär appliziert um zu testen, ob sich der Laktatwert in Abhängigkeit von der MH-Veranlagung verändert und eine Unterscheidung zwischen MHS- und MHN-Tieren erlaubt. Ziel war es, die erhobenen Daten auf eine mögliche Dosis-Wirkungs-Beziehung zu untersuchen. In der Probandenstudie wurden bei 23 Freiwilligen (neun MHS-Patienten, sieben MHN-Patienten, sieben Kontrollpersonen) über Venenverweilkatheter Mikrodialysesonden mit Zuspritzschläuchen in den M. vastus lateralis eingeführt. An die Spitze der Messsonden wurden nach einer Äquilibrierungsphase die Triggersubstanzen Halothan 6 Vol% (nur in der Kontrollgruppe), Halothan 4 Vol% und Koffein 80 mM lokal als Bolus von 200 µl über Zuspritzschläuche appliziert und die resultierenden intramuskulären Laktatveränderungen im Dialysat photospektrometrisch gemessen. Sowohl nach Stimulation mit Koffein als auch mit Halothan 4 Vol% zeigten sich Maximalwerte der Laktatkonzentrationen bei MHS-Patienten, die sich als signifikant höher als die der MHN- und Kontrollgruppe erwiesen. In der Kontrollgruppe wurde eine zusätzliche Messung mit einer Konzentration von Halothan 6 Vol% durchgeführt. Bei den Messungen wurden Werte erreicht, die für MHS-Patienten typisch sind. Im Tiermodell wurden bei gleichem Versuchsaufbau Messsonden in den Adduktorenmuskeln der Hinterläufe von neun MHS- und sechs MHN-Pietrain-Schweinen platziert. Als Triggersubstanz wurde Sevofluran in den Konzentrationen 3%, 7,5%, 15% und 28 Vol% gelöst in Sojabohnenöl als Bolus von 100 µl appliziert. Die Laktat- und Pyruvatwerte sowie die Laktat-Pyruvat-Quotienten stiegen dosisabhängig an. Es war ein signifikanter Unterschied zwischen den Maximalwerten der MHS- und MHN-Tiere nachweisbar. Die Vitalparameter wurden in beiden Versuchen kontinuierlich überwacht und metabolische Parameter vor und nach den Untersuchungen bestimmt. Es traten weder bei den untersuchten Probanden noch bei den Versuchstieren signifikante systemische oder lokale Nebenwirkungen auf. Wie schon vorhergehende Untersuchungen belegen diese beiden Studien, dass die intramuskuläre Stimulation mit MH-Triggersubstanzen zu einer Veränderung der lokalen Stoffwechselvorgänge mit signifikantem Laktatanstieg führt, welche bei MH-Prädisponierten stärker ausgeprägt ist als bei Gesunden. Koffein 80 mM und Halothan 4 Vol% ermöglichen eine sehr gute Differenzierung zwischen MHS- und MHN-Probanden. Eine zu hohe Konzentration an Triggersubstanz (Halothan 6 Vol%) ruft auch in nicht-veranlagten Patienten eine lokale, MH-ähnliche Reaktion hervor. In diesem Fall kann keine Unterscheidung zwischen MHS- und MHN-Individuen auf der Grundlage der Stoffwechselvorgänge getroffen werden. Bei der intramuskulären Applikation von Sevofluran ist eine Unterscheidung zwischen MHS- und MHN-Tieren über die resultierende Laktatkonzentration wiederum zu erreichen, so dass Sevofluran im Rahmen des Versuchsprotokolls als Ersatz für Halothan geeignet ist. Es ergibt sich eine klassische Dosis-Wirkungs-Beziehung für diese Substanz. Die von uns erhobenen Daten zeigen, dass mittels des im Rahmen dieser Arbeit eingesetzten Untersuchungsverfahrens eine zuverlässige Diagnostik einer MH-Veranlagung möglich ist. Das minimal-invasive Testverfahren könnte in naher Zukunft die heutige MH-Diagnostik ergänzen.
Fragestellung: Ischämische und anästhetikainduzierte Präkonditionierung bewirken am Myokard eine erhöhte Widerstandsfähigkeit gegen Ischämie und Reperfusion. Für diese Arbeit wurde untersucht, ob sich der Effekt ischämischer Präkonditionierung durch verlängerte oder repetitive Applikation der Ischämie verstärken lässt. Desweiteren wurde untersucht, ob sich die Präkonditionierung mit Desfluran durch Verabreichung einer höheren Konzentration oder verlängerte bzw. repetitive Applikation verstärken lässt. Methodik: In einem akuten in vivo Herzinfarktmodell des weißen Neuseelandkaninchens wurden folgenden Experimente durchgeführt. Alle Versuchstiere durchliefen eine Koronararterienokklusion von 30 min mit anschließender Reperfusion von 180 min. Zur kontinuierlichen ischämischen Präkondtionierung erhielten die Interventionsgruppen zuvor 2 min, 3 min, 5 min bzw. 15 min Ischämie. Zur repetitiven ischämischen Präkonditionierung erhielten sie zwei bzw. drei einminütige oder drei fünfminütige Zyklen Ischämie getrennt von ebenso langen Reperfusionsphasen. Zur anästhetikainduzierten Präkonditionierung erhielten die Versuchstieren vor Ischämie und Reperfusion Desfluran. Entweder kontinuierlich über 30 min oder 90 min oder repetitiv über drei zehnminütige Zyklen getrennt von ebenso langen Abflutungsphasen jeweils in Konzentrationen von 0,5, 1,0 und 1,5 MAC. Im Anschluss an die Experimente wurden die Infarktgrößen gemessen und als prozentualer Anteil des ischämischen Risikoareals dargestellt. Ergebnisse: In der Kontrollgruppe betrug die Infarktgröße 61%. 5 min Ischämie konnten eine Präkonditonierung bewirken (23%). Weder die Behandlung mit 15 min (27%) Ischämie noch mit drei fünfminütigen Zyklen (12%) waren signifikant effektiver als die einfache fünfminütige. Kontinuierliche Ischämien von 2 min (49%) bzw. 3 min (47%) senkten die Infarktgröße nicht. Zwei bzw. drei einminütigen Zyklen wirkten dagegen präkonditionierend (jew. 34%). 1,0 MAC Desfluran über 30min verabreicht senkte die Infarktgröße (35%). Weder eine höhere Konzentration von 1,5 MAC (40%) noch deren Verabreichnung über 90 min (32%) waren signifikant effektiver als 1,0 MAC. 0,5 MAC wirkte weder über 30 min (52%) noch über 90 min (56%) verabreicht präkonditionierend. Eine repetitive Verabreichung über drei zehnminütige Zyklen bewirkte dagegen Präkonditionierung (36%). Zusammnefassung: Die kardioprotektiven Effekte von kontinuierlicher ischämischer bzw. anästhetikainduzierter Präkonditionierung lassen sich oberhalb ihrer jeweiligen Reizschwelle nicht mehr verstärken. Dagegen lassen sich an sich unterschwellige Reize (Ischämie < 5 min bzw. 0,5 MAC Desfluran) durch repetitive Applikation über die Reizschwelle heben und wirken dann präkonditionierend.
Background
Approximately one in three patients suffers from preoperative anaemia. Even though haemoglobin is measured before surgery, anaemia management is not implemented in every hospital.
Objective
Here, we demonstrate the implementation of an anaemia walk‐in clinic at an Orthopedic University Hospital. To improve the diagnosis of iron deficiency (ID), we examined whether reticulocyte haemoglobin (Ret‐He) could be a useful additional parameter.
Material and Methods
In August 2019, an anaemia walk‐in clinic was established. Between September and December 2019, major orthopaedic surgical patients were screened for preoperative anaemia. The primary endpoint was the incidence of preoperative anaemia. Secondary endpoints included Ret‐He level, red blood cell (RBC) transfusion rate, in‐hospital length of stay and anaemia at hospital discharge.
Results
A total of 104 patients were screened for anaemia. Preoperative anaemia rate was 20.6%. Intravenous iron was supplemented in 23 patients. Transfusion of RBC units per patient (1.7 ± 1.2 vs. 0.2 ± 0.9; p = 0.004) and hospital length of stay (13.1 ± 4.8 days vs. 10.6 ± 5.1 days; p = 0.068) was increased in anaemic patients compared to non‐anaemic patients. Ret‐He values were significantly lower in patients with ID anaemia (33.3 pg [28.6–40.2 pg]) compared to patients with ID (35.3 pg [28.9–38.6 pg]; p = 0.015) or patients without anaemia (35.4 pg [30.2–39.4 pg]; p = 0.001).
Conclusion
Preoperative anaemia is common in orthopaedic patients. Our results proved the feasibility of an anaemia walk‐in clinic to manage preoperative anaemia. Furthermore, our analysis supports the use of Ret‐He as an additional parameter for the diagnosis of ID in surgical patients.
Takotsubo syndrome (TTS), a transient form of dysfunction in the heart's left ventricle, occurs predominantly in postmenopausal women who have emotional stress. Earlier studies support the concept that the human circulatory system is modulated by a cortical network (consisting of the anterior cingulate gyrus, amygdala, and insular cortex (Ic)) that plays a pivotal role in the central autonomic nervous system in relation to emotional stressors. The Ic plays a crucial role in the sympathovagal balance, and decreased levels of female sex hormones have been speculated to change functional cerebral asymmetry, with a possible link to autonomic instability. In this review, we focus on the Ic as an important moderator of the human brain–heart axis in association with sex hormones. We also summarize the current knowledge regarding the sex-specific neuroanatomy in TTS.

In der vorliegenden experimentellen Studie wurde die visuelle Aufmerksamkeit von Anästhesisten während der Einleitung einer Allgemeinanästhesie in Abhängigkeit von ihrer Erfahrung mithilfe eines mobilen Eye-Tracking-Gerätes untersucht. 12 Assistenten mit durchschnittlich 1,3 Jahren klinischer Erfahrung und ebenfalls 12 Fachärzte mit durchschnittlich circa 10 Jahren klinischer Erfahrung nahmen an der Studie teil. Ebenso wurde der simulierte Fall mit der Anästhesieeinleitung am Patienten verglichen. Beide Anästhesisten-Gruppen führten mit der Eye-Tracking-Brille jeweils eine Einleitung so-wohl an einem Simulator im Simulationsraum als auch am Patienten im Einleitungsraum vor dem OP-Saal durch. Diese Brille zeichnete dabei Daten zu Fixationspunkten und Blickbewegungen der Probanden auf. Angelehnt an Schulz et al.21 wurden 24 Areas of Interest definiert und diese den Gruppen „manuelle Tätigkeiten“, „Überwachung“ und „weitere Beobachtungspunkte“ zugeordnet. Als abhängige Variable für die statistische Auswertung wurde die relative Verweildauer in Prozent genutzt und eine multivariate ANOVA gerechnet.
Es ergab sich im Wesentlichen kein signifikanter Effekt des Faktors Erfahrung. Die Assistenten verteilten ihre visuelle Aufmerksamkeit nicht signifikant anders als die erfahreneren Anästhesisten und verhielten sich folglich in Bezug darauf weitgehend gleich. Der Erwerb dieses Verhaltens findet demnach anscheinend schon sehr früh statt, sodass mit 1,3 Jahren Erfahrung in dieser Hinsicht kein Unterschied zu deutlich mehr Erfahrung auftritt. Ein halbstandardisiertes Interview stützt diese Annahme. Der Faktor Umgebung beeinflusste aber signifikant das Verhalten der Anästhesisten. In der Simula-tion widmeten beide Gruppen sowohl der „Überwachung“ insgesamt als auch „manu-ellen Tätigkeiten“ während der Vorbereitungsphase mehr visuelle Aufmerksamkeit als in der Narkoseeinleitung am Patienten. Das ist einerseits mit den Einschränkungen der Simulationspuppe zu erklären und liegt andererseits wahrscheinlich an der ungewohn-ten Umgebung und der unbekannten Pflegekraft, sodass die Anästhesisten das Material länger kontrollierten. Folglich schauten beide Gruppen während der Narkoseeinleitung am Patienten häufiger auf „weitere Beobachtungspunkte“, was auch durch die höhere Komplexität und Störanfälligkeit dieser Situation schlüssig wirkt.
Optimierungsmöglichkeiten der Bedingungen im Simulationsraum werden angespro-chen, Anschlussstudien vorgeschlagen. Es konnte gezeigt werden, dass der Einsatz eines Eye-Tracking-Gerätes auch während der hochsensiblen Einleitung einer Allgemein-anästhesie am Patienten problemlos möglich ist. Darüber hinaus liefert die Studie nun eine stark standardisierte Versuchsmethode, welche für weitere Forschungsfragen sehr empfohlen werden kann.
Health economics of Patient Blood Management: a cost‐benefit analysis based on a meta‐analysis

Background and Objectives
Patient Blood Management (PBM) is the timely application of evidence‐based medical and surgical concepts designed to improve haemoglobin concentration, optimize haemostasis and minimize blood loss in an effort to improve patient outcomes. The focus of this cost‐benefit analysis is to analyse the economic benefit of widespread implementation of a multimodal PBM programme.
Materials and Methods
Based on a recent meta‐analysis including 17 studies (>235 000 patients) comparing PBM with control care and data from the University Hospital Frankfurt, a cost‐benefit analysis was performed. Outcome data were red blood cell (RBC) transfusion rate, number of transfused RBC units, and length of hospital stay (LOS). Costs were considered for the following three PBM interventions as examples: anaemia management including therapy of iron deficiency, use of cell salvage and tranexamic acid. For sensitivity analysis, a Monte Carlo simulation was performed.
Results
Iron supplementation was applied in 3·1%, cell salvage in 65% and tranexamic acid in 89% of the PBM patients. In total, applying these three PBM interventions costs €129·04 per patient. However, PBM was associated with a reduction in transfusion rate, transfused RBC units per patient, and LOS which yielded to mean savings of €150·64 per patient. Thus, the overall benefit of PBM implementation was €21·60 per patient. In the Monte Carlo simulation, the cost savings on the outcome side exceeded the PBM costs in approximately 2/3 of all repetitions and the total benefit was €1 878 000 in 100·000 simulated patients.
Conclusion
Resources to implement a multimodal PBM concept optimizing patient care and safety can be cost‐effectively.
Durch die Fussballweltmeisterschaft 2006 wurden erstmals in Deutschland Planungen für einen Massenanfall von bis zu 1000 Verletzten aufgegriffen. Um die Verteilung dieser Vielzahl von Patienten in geeignete Kliniken zu optimieren, wurden die Aufnahmekapazitäten aller bayerischen Krankenhäuser im Normalfall, bei einem Massenanfall von Verletzten sowie im Katastrophenfall untersucht. Von den 361 angeschriebenen Kliniken wurden die Angaben von 355 Häusern ausgewertet. Die erhobenen Daten wurden in einen Krankenhausatlas eingefügt, der die entsprechenden Kapazitäten in verschiedenen Entfernungskreisen um die beiden WM-Stadien München und Nürnberg in Abhängigkeit der Einsatzbereitschaften der Kliniken darstellt. Auf der Grundlage dieses Krankenhausatlasses wurde von der Münchener bzw. Nürnberger Berufsfeuerwehr ein Konzept in Form eines Wellenplanes entwickelt, welches im Falle eines Schadensereignisses an den beiden Spielstätten in den bayerischen Austragungsorten die koordinierte Verteilung von Verletzten auf umliegende Krankenhäuser gewährleistet, ohne dabei einzelne Kliniken zu überfrachten. Aber auch über die WM hinaus kann das Prinzip dieses Konzeptes für zukünftige Planungen auf lokaler Ebene beispielhaft sein und als positive Auswirkung der detaillierten Planungen dieses Großereignisses angesehen werden.
Die Bauchlagerung von intubierten ARDS-Patient/innen mit einer schlechten Oxygenierung wird laut Leitlinie seit mehreren Jahren als supportive Therapiemaßnahme empfohlen. Im Rahmen der COVID-19 Pandemie wurde nun erstmalig die Bauchlagerung auch bei hypoxämischen, nicht-intubierten Patient/innen untersucht. Diese Fragestellung wurde in der vorliegenden Arbeit mittels einer systematischen Übersichtsarbeit betrachtet. Aufgrund der aktuellen Pandemiesituation wurden neben ARDS-Patient/innen im Allgemeinen insbesondere COVID-19 Patient/innen mit einem akuten Lungenversagen als Subgruppe untersucht.
Am 21.11.2020 wurde eine systematische Suche nach Studien in den Datenbanken MEDLINE, Cochrane COVID-19 Study Register und Living Overview of the Evidence platform durchgeführt. Die Ergebnisse wurden, wo möglich, in Form einer Meta-Analyse zusammengefasst, in Tabellen darstellt oder deskriptiv beschrieben. Das Risiko für Bias wurde jeweils für die eingeschlossenen kontrollierten Studien mittels ROBINS-I beurteilt. Die Vertrauenswürdigkeit der Evidenz der gesamten Arbeit wurde mit Hilfe des GRADE-Ansatzes untersucht.
Insgesamt wurden 30 Studien eingeschlossen, davon 4 kontrollierte Studien, keine RCTs. In 3 der kontrollierten Studien wurde die Bauchlagerung bei COVID-19 Patient/innen untersucht, in einer bei Patient/innen mit einem anderweitig verursachten ARDS. Es ist unklar, ob die Bauchlagerung die Intubationsrate (RR = 0,92; 95% KI: 0,59 - 1,44; I² = 65%; sehr niedrige Vertrauenswürdigkeit der Evidenz), die Mortalität (RR = 0,55; 95% KI: 0,23 - 1,30; I² = 60%; sehr niedrige Vertrauenswürdigkeit der Evidenz) und die Wahrscheinlichkeit für eine Aufnahme auf die Intensivstation (RR = 0,94; 95% KI: 0,54 - 1,63; I2 = 71%; sehr niedrige Vertrauenswürdigkeit der Evidenz) verringern kann. Auch für die anderen betrachteten Endpunkte konnte kein signifikanter Effekt der Bauchlagerung nachgewiesen werden Im Vergleich der Subgruppen „Nicht-COVID-19“ (8 Studien) und „COVID-19“ (22 Studien) konnten in Bezug auf alle betrachteten Endpunkte keine relevanten Unterschiede festgestellt werden.
Insgesamt ist die Evidenz nicht ausreichend, um Vor- und Nachteile der Bauchlagerung für nicht-intubierte ARDS Patient/innen gegenüber der üblichen Rückenlagerung aufzuzeigen und diese für die Praxis zu empfehlen.
Background
The clinical significance of vitamin D administration in critically ill patients remains inconclusive. The purpose of this systematic review with meta-analysis was to investigate the effect of vitamin D and its metabolites on major clinical outcomes in critically ill patients, including a subgroup analysis based on vitamin D status and route of vitamin D administration.
Methods
Major databases were searched through February 9, 2022. Randomized controlled trials of adult critically ill patients with an intervention group receiving vitamin D or its metabolites were included. Random-effect meta-analyses were performed to estimate the pooled risk ratio (dichotomized outcomes) or mean difference (continuous outcomes). Risk of bias assessment included the Cochrane tool for assessing risk of bias in randomized trials.
Results
Sixteen randomized clinical trials with 2449 patients were included. Vitamin D administration was associated with lower overall mortality (16 studies: risk ratio 0.78, 95% confidence interval 0.62–0.97, p = 0.03; I2 = 30%), reduced intensive care unit length of stay (12 studies: mean difference − 3.13 days, 95% CI − 5.36 to − 0.89, n = 1250, p = 0.006; I2 = 70%), and shorter duration of mechanical ventilation (9 studies: mean difference − 5.07 days, 95% CI − 7.42 to − 2.73, n = 572, p < 0.0001; I2 = 54%). Parenteral administration was associated with a greater effect on overall mortality than enteral administration (test of subgroup differences, p = 0.04), whereas studies of parenteral subgroups had lower quality. There were no subgroup differences based on baseline vitamin D levels.
Conclusions
Vitamin D supplementation in critically ill patients may reduce mortality. Parenteral administration might be associated with a greater impact on mortality. Heterogeneity and assessed certainty among the studies limits the generalizability of the results.
Es handelt sich um eine randomisierte, kontrollierte und einfach verblindete Studie, um Ultraschall und Nervenstimulation bei der axillären Plexusblockade (Multiinjektionstechnik) bezüglich Anschlagszeiten, Durchführungszeit, Blockadeerfolg, Komplikationsraten und Patientenkomfort zu vergleichen. Es ergaben sich nur kleine bzw. keine signifikanten Unterschiede in den Zeiten, Erfolgsraten, Komplikationsraten und beim Patientenkomfort. Bei gleicher klinischer Eignung ist die sonographische Methode zu favorisieren.
(1) Background: Nowadays, the use of microsurgical free flaps is a standard operative procedure in reconstructive surgery. Still, thrombosis of the microanastomosis is one of the most fatal postoperative complications. Clinical evaluation, different technical devices and laboratory markers are used to monitor critical flap perfusion. Macrophage migration inhibitory factor (MIF), a structurally unique cytokine with chemokine-like characteristics, could play a role in predicting vascular problems and the failure of flap perfusion. (2) Methods: In this prospective observational study, 26 subjects that underwent microsurgical reconstruction were observed. Besides clinical data, the number of blood leukocytes, CRP and MIF were monitored. (3) Results: Blood levels of MIF, C-reactive protein (CRP) and leukocytes increased directly after surgery. Subjects that needed surgical revision due to thrombosis of the microanastomosis showed significantly higher blood levels of MIF than subjects without revision. (4) Conclusion: We conclude that MIF is a potential and innovative indicator for thrombosis of the microanastomosis after free flap surgery. Since it is easy to obtain diagnostically, MIF could be an additional tool to monitor flap perfusion besides clinical and technical assessments.
Non-steroidal antiinflammatory drugs are most commonly used for inflammatory and postoperative pain. But they lack effectiveness and specificity, leading to severe side effects, like gastric ulcers, asthma and severe bleeding. Oxidized 1-palmitoyl-2-arachinidonoyl-sn-glycero-3-phosphocholine (OxPAPC) plays an important role in inflammatory pain. PAPC is a common phosphatidylcholine of membranes, which can be oxidized by reactive oxygen species. In preliminary experiments, our group found that local injection of OxPAPC in rat paws induces hyperalgesia.
In this study we examined the effect of OxPAPC on transient receptor potential A1 (TRPA1), an ion channel expressed in C-fiber neurons. Furthermore, we investigated if intracellular cysteine residues of TRPA1 were necessary for agonist-channel-interactions and if a subsequent TRPA1 activation could be prevented by OxPAPC scavengers.
To answer these questions, we performed calcium imaging using HEK-293 cells stably expressing hTRPA1, or transiently expressing the triple mutant channel hTRPA1-3C and naïve DRG neurons. Cells were incubated with the ratiometric, fluorescent dye Fura-2/AM and stimulated with OxPAPC. The change of light emission after excitation with 340 and 380 nm wavelengths allowed conclusions regarding changes of intracellular calcium concentrations after TRPA1 activation.
In our investigation we proved evidence that OxPAPC activates TRPA1, which caused a flow of calcium ions into the cytoplasm. The TRPA1-specific channel blocker HC-030031 eliminated this agonist-induced response. TRPA1-3C was not completely sensitive to OxPAPC. The peptide D-4F and the monoclonal antibody E06 neutralized OxPAPC-induced TRPA1 activation.
In this work, the importance of OxPAPC as a key mediator of inflammatory pain and as a promising target for drug design is highlighted. Our results indicate that TRPA1 activation by OxPAPC involves cysteine-dependent mechanisms, but there are other, cysteine-independent activation mechanisms as well. Potential pharmaceuticals for the treatment of inflammatory pain are D-4F and E06, whose efficiency has recently been confirmed in the animal model by our research group.
PONV ist eine häufige und für Patient*innen belastende Nebenwirkung nach einer Allgemeinanästhesie. Trotz der Vielzahl an Studien zu den zahlreichen antiemetischen Medikamenten gibt es bisher keinen Überblick über die Effizienz und Sicherheit all dieser Medikamente. Im Rahmen des Cochrane-Reviews „Drugs for preventing postoperative nausea and vomiting in adults after general anaesthesia: a network meta-analysis“ wurden RCTs zur Prävention von PONV nach Allgemeinanästhesie bei Erwachsenen gesucht. Zu den primären Endpunkten gehörten Erbrechen 0-24 Stunden, schwere unerwünschte Ereignisse und unerwünschte Ereignisse, zu den sekundären Endpunkten Substanz-spezifische Nebenwirkungen, frühes und spätes postoperatives Erbrechen, Übelkeit und vollständiger Behandlungserfolg. In dieser Dissertation wurden die Vergleiche mit mindestens zehn Studien auf das Vorliegen eines Publikationsbias überprüft. Die Beurteilung des Publikationsbias erfolgte unter Anwendung verschiedener Tests (Funnel Plots, contour-enhanced Funnel Plot, Arcsine Test, Trim-and-Fill-Methode). Bei sieben von den 64 analysierten Vergleichen wurde der Verdacht auf einen Publikationsbias gestellt. Für den primären Endpunkt Erbrechen 0-24 Stunden wurde bei zwei Vergleichen (Droperidol vs. Placebo und Metoclopramid vs. Ondansetron) ein Publikationsbias vermutet, für die sekundären Endpunkte Übelkeit bei drei Vergleichen (Tropisetron vs. Placebo, Dexamethason-Ondansetron vs. Dexamethason, Dexamethason-Ondansetron vs. Ondansetron) und für den vollständigen Behandlungserfolg bei zwei Vergleichen (Droperidol vs. Placebo, Ondansetron vs. Placebo). Die Effektschätzer der restlichen 54 Vergleiche sind hinsichtlich der klinischen Relevanz robust und eine Verzerrung durch einen Publikationsbias wurde nicht vermutet.
Das Ziel der vorliegenden Arbeit war, die aktuelle Primärliteratur zum Thema Diagnostik der Hypovolämie zu einer Metastudie zusammenzufassen. Darauf aufbauend sollte analysiert werden, ob bereits verlässliche Methoden und Parameter zur Bestimmung des Volumenstatus verfügbar sind. Nach Durchsicht und Analyse der Datenlage zur Diagnostik der Hypovolämie in der aktuellen Primärliteratur im Zeitraum von 1995-2005 zeigte sich, dass die in die Studie eingeschlossenen Arbeiten nicht vergleichbar und nicht als Metastudie zusammenzufassen sind. Gründe hierfür sind die große Heterogenität im Versuchsaufbau, der detaillierten Versuchsdurchführung und der verwendeten Methoden und Parameter zur Bestimmung des Blutvolumens. Die Analyse der Primärliteratur zeigte, dass bisher keine verlässlichen Parameter zur Bestimmung des Blutvolumens und zur frühzeitigen Erkennung einer Hypovolämie existieren. Im klinischen Routinebetrieb erscheint die transpulmonalen Thermodilution (PiCCO) das derzeit sensitivste Verfahren zur Volumenbestimmung zu sein. Auf Grund der mangelnden Spezifität sind Fehleinschätzungen der tatsächlichen Volumensituation auch bei dieser Technik häufig. Die zusätzliche Durchsicht der nach 2005 erschienen Arbeiten machte deutlich, dass die Methoden zur Volumenbestimmung zwar verbessert wurden, trotzdem aber noch etlichen Limitationen unterliegen, die einen verlässlichen Einsatz im klinischen Alltag zur sicheren Bestimmung der Volumenssituation nicht erlauben. Es sind daher weitere experimentelle und klinische Arbeiten auf diesem Gebiet nötig, um verlässliche Parameter und Methoden zur Volumendiagnostik zu schaffen.
Functional and structural characterization of axonal opioid receptors as targets for analgesia

Background
Opioids are the gold standard for the treatment of acute pain despite serious side effects in the central and enteric nervous system. µ-opioid receptors (MOPs) are expressed and functional at the terminals of sensory axons, when activated by exogenous or endogenous ligands. However, the presence and function of MOP along nociceptive axons remains controversial particularly in naïve animals. Here, we characterized axonal MOPs by immunofluorescence, ultrastructural, and functional analyses. Furthermore, we evaluated hypertonic saline as a possible enhancer of opioid receptor function.
Results
Comparative immunolabeling showed that, among several tested antibodies, which all provided specific MOP detection in the rat central nervous system (CNS), only one monoclonal MOP-antibody yielded specificity and reproducibility for MOP detection in the rat peripheral nervous system including the sciatic nerve. Double immunolabeling documented that MOP immunoreactivity was confined to calcitonin gene-related peptide (CGRP) positive fibers and fiber bundles. Almost identical labeling and double labeling patterns were found using mcherry-immunolabeling on sciatic nerves of mice producing a MOP-mcherry fusion protein (MOP-mcherry knock-in mice). Preembedding immunogold electron microscopy on MOP-mcherry knock-in sciatic nerves indicated presence of MOP in cytoplasm and at membranes of unmyelinated axons. Application of [D-Ala\(^2\), N-MePhe\(^4\), Gly-ol]-enkephalin (DAMGO) or fentanyl dose-dependently inhibited depolarization-induced CGRP release from rat sciatic nerve axons ex vivo, which was blocked by naloxone. When the lipophilic opioid fentanyl was applied perisciatically in naïve Wistar rats, mechanical nociceptive thresholds increased. Subthreshold doses of fentanyl or the hydrophilic opioid DAMGO were only effective if injected together with hypertonic saline. In vitro, using β-arrestin-2/MOP double-transfected human embryonic kidney cells, DAMGO as well as fentanyl lead to a recruitment of β-arrestin-2 to the membrane followed by a β-arrestin-2 reappearance in the cytosol and MOP internalization. Pretreatment with hypertonic saline prevented MOP internalization.
Conclusion
MOPs are present and functional in the axonal membrane from naïve animals. Hypertonic saline acutely decreases ligand-induced internalization of MOP and thereby might improve MOP function. Further studies should explore potential clinical applications of opioids together with enhancers for regional analgesia.
Hintergrund:
Die tatsächliche Ankunftszeit von schwerverletzten oder kritisch kranken Patienten im Schockraum einer Klinik stimmt nicht immer mit der von der Rettungsleitstelle angekündigten Ankunftszeit überein. Im Rahmen einer retrospektiven Analyse an einem deutschen überregionalen Traumazentrum wurde untersucht, ob der dortige Alarmierungsalgorithmus geeignet ist, Zeitabweichungen in der Patientenankunft zu kompensieren.
Methode:
Die Datenanalyse erfolgte retrospektiv. Es wurde die Differenz zwischen angekündigter und tatsächlicher Eintreffzeit aller über das Schockraumtelefon angekündigten und im Schockraum aufgenommenen Patienten von September 2010 bis März 2011 ermittelt. Die Teamalarmierung erfolgte 10 Minuten vor angekündigter Patientenankunft.
Ergebnisse:
In die Untersuchung wurden 165 Patienten eingeschlossen. Bei 11% aller Patienten und bei 9% der primär über den Schockraum aufgenommenen Traumapatienten stimmten angekündigte und tatsächliche Ankunftszeit überein. In 24% aller Fälle lag die tatsächliche Ankunftszeit des Patienten vor der angekündigten Ankunftszeit. 3% des gesamten Patientenkollektives und 0% aus der Gruppe der schwer betroffenen Traumapatienten kamen vor der Teamversammlung im Schockraum an. Zu Wartezeiten des Teams von über 20 Minuten kam es in 9% aller Fälle.
Schlussfolgerung:
Bei einer Teamalarmierung 10 Minuten vor angekündigter Ankunftszeit kann eine vollständige Versammlung des Schockraumteams vor Ankunft des Patienten in 97% aller Fälle erreicht werden. Gleichzeitig resultieren akzeptable Wartezeiten für das Team.
Die Durchführung der interskalenären Plexus brachialis Blockade (ISB)zusätzlich zur Allgemeinanästhesie hat sich bei Operationen im Schulterbereich zur Verbesserung der regionalen Schmerztherapie bewährt. Im Vergleich zum Standardverfahren der Nervenstimulation ist die Verwendung der Sonografie zur Identifizierung des Plexus brachialis eine neuere Methode, die eine direkte Visualisierung der Ziel- und umgebenden Strukturen ohne Notwendigkeit der elektrischen Nervenstimulation ermöglicht. In unserer prospektiven Studie, in die 500 Patienten eingeschlossen wurden, wurden die Methoden "Nervenstimulation", "Sonografie" und die Kombination beider Verfahren hinsichtlich Durchführung, Effektivität, Komplikationen und Patientenzufriedenheit verglichen. Während beide Methoden der ISB-Anlage durchschnittlich in etwa dieselbe Zeit in Anspruch nahmen, traten bei dem ultraschallgestützten Verfahren weniger häufig Schmerzen und Parästhesien auf. Das kombinierte Verfahren nahm deutlich mehr Zeit in Anspruch und löste am häufigsten Parästhesien aus. Der Blockadeerfolg war in der Gruppe „Nervenstimulation“ größer als in der Ultraschallgruppe, wobei der Unterschied nicht signifikant war. Die Effektivität der interskalenären Blockaden, gemessen an der postoperativen Schmerzintensität, der Substanzklasse der ersten Schmerztherapie, der Aufenthaltszeit im Aufwachraum und der Wirkdauer der Nervenblockade zeigte keinen statistisch relevanten Unterschied zwischen den Gruppen. Nur die Zeitdauer von der Freigabe bis zur ersten postoperativen Schmerzmedikation war bei den mithilfe der Nervenstimulation angelegten ISBs signifikant länger als in den beiden anderen Gruppen. Bezüglich des Auftretens von neurologischen oder respiratorischen Komplikationen nach der Blockade sowie hinsichtlich der Patientenzufriedenheit ließ sich kein Unterschied zwischen den Blockadeverfahren nachweisen. Der wichtigste Vorteil der sonogesteurten Technik ist die direkte Visualisierung der anatomischen Strukturen und der Injektion und Verteilung des Lokalanästhetikums. Dadurch können schwerwiegende Komplikationen vermieden werden. Durch den Verzicht auf elektrische Nervenstimulation ist die ISB-Anlage für den Patienten angenehmer. Unsere Ergrebnisse zeigen, dass die sonografisch gesteuerte Anlage von interskalenären Blockaden eine gleichwertige Alternative zur Nervenstimulation darstellt.
Purpose
Therapeutic options for breast cancer (BC) treatment are constantly evolving. The Human Epidermal Growth Factor 2 (HER2)-low BC entity is a new subgroup, representing about 55% of all BC patients. New antibody–drug conjugates demonstrated promising results for this BC subgroup. Currently, there is limited information about the conversion of HER2 subtypes between primary tumor and recurrent disease.
Methods
This retrospective study included women with BC at the University Medical Centre Wuerzburg from 1998 to 2021. Data were retrieved from patients' records. HER2 evolution from primary diagnosis to the first relapse and the development of secondary metastases was investigated.
Results
In the HR-positive subgroup without HER2 overexpression, HER2-low expression in primary BC was 56.7 vs. 14.6% in the triple-negative subgroup (p < 0.000). In the cohort of the first relapse, HER2-low represented 64.1% of HR-positive vs. 48.2% of the triple-negative cohort (p = 0.03). In patients with secondary metastases, HER2-low was 75.6% vs. 50% in the triple negative subgroup (p = 0.10). The subgroup of HER2-positive breast cancer patients numerically increased in the course of disease; the HER2-negative overall cohort decreased. A loss of HER2 expression from primary BC to the first relapse correlated with a better OS (p = 0.018). No clinicopathological or therapeutic features could be identified as potential risk factors for HER2 conversion.
Conclusion
HER2 expression is rising during the progression of BC disease. In view of upcoming therapeutical options, the re-analysis of newly developed metastasis will become increasingly important.
In my thesis, I characterized aGPCRs Adgrl1 and Adgrl3, tight junction proteins and the blood-DRG-barrier in rats’ lumbar dorsal root ganglions after traumatic neuropathy. In contrast to the otherwise tightly sealed barriers shielding neural tissues, the dorsal root ganglion’s neuron rich region is highly permeable in its healthy state. Furthermore, the DRG is a source of ectopic signal generation during neuropathy; the exact origin of which is still unclear. I documented expression of Adgrl1 and Adgrl3 in NF200 + , CGRP + and IB4 + neurons. One week after CCI, I observed transient downregulation of Adgrl1 in non-peptidergic nociceptors (IB4+). In the context of previous data, dCirl deletion causing an allodynia-like state in Drosophila, our research hints to a possible role of Adgrl1 nociceptive signal processing and pain resolution in neuropathy. Furthermore, I demonstrated similar claudin-1, claudin-12, claudin-19, and ZO-1 expression of the dorsal root ganglion’s neuron rich and fibre rich region. Claudin-5 expression in vessels of the neuron rich region was lower compared to the fibre rich region. Claudin-5 expression was decreased one week after nerve injury in vessels of the neuron rich region while permeability for small and large injected molecules remained unchanged. Nevertheless, we detected more CD68+ cells in the neuron rich region one week after CCI. As clinically relevant conclusion, we verified the high permeability of the neuron rich regions barrier as well as a vessel specific claudin-5 downregulation after CCI. We observed increased macrophage invasion into the neuron rich region after CCI. Furthermore, we identified aGPCR as potential target for further research and possible treatments for neuropathy, which should be easily accessible due to the blood-DRG-barriers leaky nature. Its precise function in peripheral tissues, its mechanisms of activation, and its role in pain resolution should be evaluated further.
The nervous system is shielded by special barriers. Nerve injury results in blood–nerve barrier breakdown with downregulation of certain tight junction proteins accompanying the painful neuropathic phenotype. The dorsal root ganglion (DRG) consists of a neuron-rich region (NRR, somata of somatosensory and nociceptive neurons) and a fibre-rich region (FRR), and their putative epi-/perineurium (EPN). Here, we analysed blood–DRG barrier (BDB) properties in these physiologically distinct regions in Wistar rats after chronic constriction injury (CCI). Cldn5, Cldn12, and Tjp1 (rats) mRNA were downregulated 1 week after traumatic nerve injury. Claudin-1 immunoreactivity (IR) found in the EPN, claudin-19-IR in the FRR, and ZO-1-IR in FRR-EPN were unaltered after CCI. However, laser-assisted, vessel specific qPCR, and IR studies confirmed a significant loss of claudin-5 in the NRR. The NRR was three-times more permeable compared to the FRR for high and low molecular weight markers. NRR permeability was not further increased 1-week after CCI, but significantly more CD68\(^+\) macrophages had migrated into the NRR. In summary, NRR and FRR are different in naïve rats. Short-term traumatic nerve injury leaves the already highly permeable BDB in the NRR unaltered for small and large molecules. Claudin-5 is downregulated in the NRR. This could facilitate macrophage invasion, and thereby neuronal sensitisation and hyperalgesia. Targeting the stabilisation of claudin-5 in microvessels and the BDB barrier could be a future approach for neuropathic pain therapy.
Pharmacologic cardiac conditioning increases the intrinsic resistance against ischemia and reperfusion (I/R) injury. The cardiac conditioning response is mediated via complex signaling networks. These networks have been an intriguing research field for decades, largely advancing our knowledge on cardiac signaling beyond the conditioning response. The centerpieces of this system are the mitochondria, a dynamic organelle, almost acting as a cell within the cell. Mitochondria comprise a plethora of functions at the crossroads of cell death or survival. These include the maintenance of aerobic ATP production and redox signaling, closely entwined with mitochondrial calcium handling and mitochondrial permeability transition. Moreover, mitochondria host pathways of programmed cell death impact the inflammatory response and contain their own mechanisms of fusion and fission (division). These act as quality control mechanisms in cellular ageing, release of pro-apoptotic factors and mitophagy. Furthermore, recently identified mechanisms of mitochondrial regeneration can increase the capacity for oxidative phosphorylation, decrease oxidative stress and might help to beneficially impact myocardial remodeling, as well as invigorate the heart against subsequent ischemic insults. The current review highlights different pathways and unresolved questions surrounding mitochondria in myocardial I/R injury and pharmacological cardiac conditioning.
Präklinisches Management bei Herz-Kreislauf-Stillstand – extrakorporale kardiopulmonale Reanimation

Die Überlebenschancen nach präklinischem Herz-Kreislauf-Stillstand sind weiterhin sehr gering. Trotz intensiver Bemühungen bleibt das Outcome seit vielen Jahren weitestgehend konstant. Neue Technologien wie die extrakorporale kardiopulmonale Reanimation (eCPR) können in bestimmten Situationen möglicherweise das Überleben mit gutem neurologischen Outcome signifikant verbessern.
Fragestellung
Beeinflusst die sofortige Reperfusion und Reoxygenierung des Körpers mittels eCPR das Überleben nach Herz-Kreislauf-Stillstand? Bedarf es einer Erweiterung der „chain of survival“ um die eCPR?

Ergebnisse
Die Überlebensraten nach präklinischem Herz-Kreislauf-Stillstand sind weltweit seit vielen Jahren unverändert bei 10–30 %. Trotz geringer Fallzahlen zeigen neuere retrospektive Studien, dass durch die eCPR eine Verbesserung des Outcome erzielt werden kann. In selektionierten Patientenkollektiven ist ein Überleben mit gutem neurologischen Outcome von 38 % möglich.
Schlussfolgerung
Ob und mit welcher Lebensqualität ein Herz-Kreislauf-Stillstand überlebt werden kann, ist von vielen unterschiedlichen Faktoren abhängig. Der Faktor Zeit, also die Vermeidung einer „No-flow-Phase“ und die Reduktion der „Low-flow-Phase“, ist von zentraler Bedeutung. Durch die sofortige Wiederherstellung von Zirkulation und Sauerstoffversorgung kann durch die eCPR das Überleben signifikant verbessert werden. Große kontrollierte, randomisierte Studien hierzu fehlen jedoch bisher.
Background
Increasing numbers of patients receiving oral anticoagulants are undergoing elective surgery. Low molecular weight heparin (LMWH) is frequently applied as bridging therapy during perioperative interruption of anticoagulation. The aim of this study was to explore the postoperative bleeding risk of patients receiving surgery under bridging anticoagulation.
Methods
We performed a monocentric retrospective two-arm matched cohort study. Patients that received perioperative bridging anticoagulation were compared to a matched control group with identical surgical procedure, age, and sex. Emergency and vascular operations were excluded. The primary endpoint was the incidence of major postoperative bleeding. Secondary endpoints were minor postoperative bleeding, thromboembolic events, length of stay, and in-hospital mortality. Multivariate analysis explored risk factors of major postoperative bleeding.
Results
A total of 263 patients in each study arm were analyzed. The patient cohort included the entire field of general and visceral surgery including a large proportion of major oncological resections. Bridging anticoagulation increased the postoperative incidence of major bleeding events (8% vs. 1%; p < 0.001) as well as minor bleeding events (14% vs. 5%; p < 0.001). Thromboembolic events were equally rare in both groups (1% vs. 2%; p = 0.45). No effect on mortality was observed (1.5% vs. 1.9%). Independent risk factors of major postoperative bleeding were full-therapeutic dose of LMWH, renal insufficiency, and the procedure-specific bleeding risk.
Conclusion
Perioperative bridging anticoagulation, especially full-therapeutic dose LMWH, markedly increases the risk of postoperative bleeding complications in general and visceral surgery. Surgeons should carefully consider the practice of routine bridging.
Hypersensitivity to mechanical stimuli is a cardinal symptom of neuropathic and inflammatory pain. A reduction in spinal inhibition is generally considered a causal factor in the development of mechanical hypersensitivity after injury. However, the extent to which presynaptic inhibition contributes to altered spinal inhibition is less well established. Here, we used conditional deletion of GABA\(_A\) in NaV1.8-positive sensory neurons (Scn10a\(^{Cre}\);Gabrb3\(^{fl/fl}\)) to manipulate selectively presynaptic GABAergic inhibition. Behavioral testing showed that the development of inflammatory punctate allodynia was mitigated in mice lacking pre-synaptic GABA\(_A\). Dorsal horn cellular circuits were visualized in single slices using stimulus-tractable dual-labelling of c-fos mRNA for punctate and the cognate c-Fos protein for dynamic mechanical stimulation. This revealed a substantial reduction in the number of cells activated by punctate stimulation in mice lacking presynaptic GABA\(_A\) and an approximate 50% overlap of the punctate with the dynamic circuit, the relative percentage of which did not change following inflammation. The reduction in dorsal horn cells activated by punctate stimuli was equally prevalent in parvalbumin- and calretinin-positive cells and across all laminae I–V, indicating a generalized reduction in spinal input. In peripheral DRG neurons, inflammation following complete Freund’s adjuvant (CFA) led to an increase in axonal excitability responses to GABA, suggesting that presynaptic GABA effects in NaV1.8\(^+\) afferents switch from inhibition to excitation after CFA. In the days after inflammation, presynaptic GABA\(_A\) in NaV1.8\(^+\) nociceptors constitutes an “open gate” pathway allowing mechanoreceptors responding to punctate mechanical stimulation access to nociceptive dorsal horn circuits.
Background: Phenprocoumon has been used as an oral anticoagulant in patients with thromboembolic disease for more than 40 years. So far its pharmacokinetics have not been analyzed in emergency situations. Methods: Phenprocoumon-treated patients with major bleeding or urgent surgery were included in a prospective, observational registry. Phenprocoumon drug concentrations were analyzed in samples, collected as part of routine care using ultraperformance liquid chromatography tandem mass spectrometry. Moreover, anticoagulant intensity and drug half-life (t1/2) were calculated. Results: 115 patients were included. Phenprocoumon levels declined over time with a half-life of 5.27 and 5.29 days in patients with major bleedings (n = 82) and with urgent surgery (n = 33). Baseline phenprocoumon levels were 2.2 times higher in the bleeding group compared to the surgery group (1.92 vs. 0.87 ng/mL, p < 0.0001). International normalized ratio (INR) values decreased rapidly during the first 24 h. In 27.6% of patients a rebound of INR (recurrent increase > 1.5) was observed which was associated with significantly increased bleeding rates (22% vs. 4.2% in patients with or without INR rebound, p = 0.012). Conclusions: In emergency situations, the long half-life of phenprocoumon may cause INR rebound and associated recurrent bleedings. Optimal management may need to include repeated vitamin K supplementation over days.
In früheren Studien wurde gezeigt, dass durch eine mit FCA-induzierte Pfotenentzündung die Permeabilität für hydrophile Analgetika der kleinen Nerven am Entzündungsort zunimmt. In der vorliegenden Arbeit wurden spezifische Veränderungen von Barriereproteinen des Perineuriums und der Schwannschen Zellen und ihren Regulatoren nach intraplantarer Injektion von FCA lokal in die Hinterpfote und proximal am N. ischiadicus untersucht. Aus früheren Studien ist bekannt, dass vor allem Claudin-1 das Perineurium abdichtet. Daher konzentrierte sich die Arbeit auf Claudin-1 und einen möglichen Einfluss von Claudin-19 aus Schwannschen Zellen. Alle Untersuchungen erfolgten an Wister-Ratten.
Zwei Stunden bis 96 Stunden nach der FCA-Injektion in die Hinterpfote waren die Expression sowie die Immunreaktivität von Claudin-1 und die Expression von Claudin-19 im ipsilateralen proximalen Ischiasnerv unverändert. Zudem wurde keine Penetration des Farbstoffes EBA in das Endoneurium und in den Ischiasnerv nach ex vivo Applikation nachgewiesen, was auf eine gute Abdichtung des Perineuriums hinweist. In der entzündeten Pfote selbst allerdings nahm die Expression von Claudin-1 und Claudin-19 ab. Parallel dazu kam es zu einer starken Abnahme des Co-Transkriptionsfaktors β-Catenin in der Pfote, aber nicht im Nerven. β-Catenin steuert die Expression von Claudin-1. Die Behandlung mit einem GSK3 β-Inhibitor bremste die Herunterregulation von Claudin-1 24 Stunden nach der intraplantaren Injektion von FCA ins Hinterpfotengewebe und führte zu einem Wiederanstieg der Konzentration.
Daher kann abschließend festgehalten werden, dass eine periphere Entzündung zwar wie erwartet lokal die Barriere öffnet, es aber proximal nicht zu einer Barrierestörung kommt. Dies ist bei der Blut-Hirn-Schranke anders. Diese wird vermutlich über lösliche Faktoren bei Entzündung oder bei Nervenschäden, bei denen sich auch die Barriere im Spinalganglion verändert, durchlässiger.
Background:
Regional ventilation of the lung can be visualized by pulmonary electrical impedance tomography (EIT). The aim of this study was to examine the post‐operative redistribution of regional ventilation after lung surgery dependent on the side of surgery and its association with forced vital capacity.
Methods:
In this prospective, observational cohort study 13 patients undergoing right and 13 patients undergoing left‐sided open or video‐thoracoscopic procedures have been investigated. Pre‐operative measurements with EIT and spirometry were compared with data obtained 3 days post‐operation. The center of ventilation (COV) within a 32 × 32 pixel matrix was calculated from EIT data. The transverse axis coordinate of COV, COVx (left/right), was modified to COVx′ (ipsilateral/contralateral). Thus, COVx′ shows a negative change if ventilation shifts contralateral independent of the side of surgery. This enabled testing with two‐way ANOVA for repeated measurements (side, time).
Results:
The perioperative shift of COVx′ was dependent on the side of surgery (P = .007). Ventilation shifted away from the side of surgery after the right‐sided surgery (COVx′‐1.97 pixel matrix points, P < .001), but not after the left‐sided surgery (COVx′‐0.61, P = .425). The forced vital capacity (%predicted) decreased from 94 (83‐109)% (median [quartiles]; [left‐sided]) and 89 (80‐97)% (right‐sided surgery) to 61 (59‐66)% and 62 (40‐72)% (P < .05), respectively. The perioperative changes in forced vital capacity (%predicted) were weakly associated with the shift of COVx′.
Conclusion:
Only after right‐sided lung surgery, EIT showed reduced ventilation on the side of surgery while vital capacity was markedly reduced in both groups.
Hintergrund: Die pulmonale elektrische Impedanztomographie (EIT) stellt die regionale Ventilation der Lunge dar. Ziel dieser Studie war es, die postoperative Umverteilung der regionalen Ventilation nach lungenchirurgischen Eingriffen in Abhängigkeit von der Operationsseite und deren Zusammenhang mit der forcierten Vitalkapazität zu untersuchen.
Methoden: In dieser Beobachtungsstudie wurden Patienten untersucht, die sich rechts- und linksseitig offenen und videothorakoskopischen Eingriffen unterzogen. Es wurden Messungen mit EIT und Spirometrie durchgeführt. Jeweils 13 links- und 13 rechtsseitig operierte erwachsene Patienten, wurden präoperativ, sowie am dritten postoperativen Tag untersucht. Der Center of Ventilation (COV) innerhalb einer 32 x 32 Matrix wurde aus den EIT-Daten berechnet. COVx (links/rechts) wurde zu COVx' (ipsilateral/kontralateral) mit invertiertem Vektor der perioperativen Veränderung bei rechtsseitiger Operation modifiziert, um den Effekt der Zeit und der Seite der Operation in beiden Gruppen mit Two-way ANOVA für wiederholte Messungen zu testen.
Ergebnisse: Die perioperative Verschiebung von COVx' war abhängig von der Operationsseite (p=0,007) und war um -1,97 Matrixpunkte (p<0,001) bei rechtsseitiger und -0,61 Matrixpunkte (p=0,425) bei linksseitiger Operation von der Operationsseite weg gerichtet. Die forcierte Vitalkapazität (%vom Soll) verringerte sich von 94 (83-109) % [Median (Perzentile)] (linksseitig) und 89 (80-97) % (rechtsseitig) auf 61 (59-66) % bzw. 62 (40-72) % (p<0,05) und korrelierte mit COVx' für beide Gruppen.
Schlussfolgerung: Nur nach rechtsseitiger Lungenoperation konnte die EIT eine reduzierte Ventilation auf der Seite der Operation zeigen, während die Vitalkapazität in beiden Gruppen deutlich reduziert war.
Die suffiziente Atemwegssicherung ist eine der wichtigsten Aufgaben des Anästhesisten. Die endotracheale Intubation mit dem konventionellen Laryngoskop nach Macintosh stellt aktuell den Goldstandard dar. Gerade bei aspirationsgefährdeten Patienten sollte die Intubation zügig, aber dennoch schonend und sicher erfolgen. Die Rapid Sequence Induction als Sonderform der Narkoseeinleitung ist hier das Mittel der Wahl. Bei Vorliegen eines schwierigen Atemweges kann diese aber massiv erschwert sein. Eine relativ neue Alternative abseits der konventionellen Laryngoskopie bietet hier die Anwendung der Videolaryngoskopie. Das C-MAC der Firma Karl Storz ist ein Vertreter dieser Videolaryngoskope und erst seit wenigen Jahren auf dem Markt erhältlich. Basierend auf bisherigen Studien sollte nun im Rahmen der klinischen Anwendung des C-MAC bei Blitzintubationen eine etwaige Überlegenheit im Vergleich mit der direkten Laryngoskopie evaluiert werden.
Um diese Frage weiter zu klären, wurden in dieser Studie Daten von 60 Patienten erhoben, die vor Elektivoperationen in der Augen- und HNO-Heilkunde eine RSI benötigten. Jeweils 30 Patienten wurden randomisiert, entweder mit Hilfe des C-MAC-Videolaryngoskopes oder des konventionellen Laryngoskopes nach Macintosh, intubiert. Der Intubation mit dem Videolaryngoskop vorgeschaltet war immer eine direkte Laryngoskopie. Primärer Endpunkt der Studie war der Intubationserfolg.
Bezüglich dieses Endpunktes zeigte sich keine Überlegenheit des C-MAC, da der Intubationserfolg in beiden Gruppen exakt gleich war. Es gelangen 96,7 % der Intubationen jeweils im ersten Versuch. Hinsichtlich der Intubationsdauer bestanden keine Vorteile durch die Verwendung der Videolaryngoskopie. In der Versuchsgruppe benötigten die Intubationen im Mittel 33,2 Sekunden und damit 15,3 Sekunden länger als in der Kontrollgruppe, in der die Intubationen im Durchschnitt 17,9 Sekunden in Anspruch nahmen.
Es zeigte sich eine klinisch relevante Überlegenheit des C-MAC bezüglich der Klassifikation nach Cormack und Lehane, also eine Verbesserung der Glottisvisualisierung. Aufgrund der primär besseren Visualisierung der Stimmbandebene wurden signifikant weniger Optimierungshilfen benötigt. Einschränkend ist hierbei zu erwähnen, dass die Tubuspassage trotz optimaler Sichtverhältnisse massiv erschwert sein kann, da in der Videolaryngoskopie die orale nicht mit der pharyngealen und der laryngealen Achse übereinstimmen muss und somit eine starke Divergenz zwischen der visuellen und der anatomischen Achse zu Intubationsproblemen führen kann.
Der Schwierigkeitsgrad der Intubationen war bei Anwendung des C-MAC geringer als in der Kontrollgruppe, was sich somit in niedrigeren Werten bezüglich der Intubation Difficulty Scale widerspiegelte. Es bestanden keine signifikanten Unterschiede hinsichtlich der Vermeidung von, durch die Intubation verursachten, Schleimhautblutungen und postoperativen Beschwerden. Die beiden Laryngoskopietechniken hatten keinen unterschiedlichen Einfluss auf die hämodynamischen Parameter der Probanden.
Insgesamt sollten beide Methoden als gleichwertig betrachtet werden. Das C-MAC stellt somit eine nützliche Alternative bei der RSI dar. Aufgrund der besseren Visualisierungsmöglichkeiten eignet sich die Videolaryngoskopie besonders beim schwierigen Atemweg.
