The current study presents a new a group of Demotic ostraca in the belongings of the Cairo Museum. A large part of this group stem from Medinet Habu in the western bank of modern Luxor in Upper Egypt and was discovered in the beginning of the thirties of the last century by the Chicago Oriental Institute (recently renamed as Institute for the Study of Ancient Cultures ‘ISAC’). A small portion of the collection under consideration come from other Upper Egyptian provenances including Gebelein, Edfu, Kom Ombo, and possibly elsewhere in Thebes. The main goal of the present dissertation is to decipher, translate, and provide a philological, paleographical, and cultural analysis of the group of texts in question. The results of this study are spread over two main parts, the first of which is dedicated to the main and largest part of the collection, i.e. ostraca from Medinet Habu, while the second is concerned with ostraca from other places. The first part comprises of five sections beginning with receipts of money and in-kind payments including some receipts for the payments of the different capitation charges in the Ptolemaic and Roman Periods, a few for land-related payments, as well as others related to different Ptolemaic monopolies or trades such as a receipt for the price of oil, one for the linen tax, in addition to a unique receipt for the rarely attested fish tax. The second section includes accounts and lists of different kinds be it monetary, in-kind, agriculture, or any other type of lists or accounts that record different everyday transactions. The following section presents a relatively different type of lists, namely lists of personal names. The fourth section incorporates a variety of texts of different concerns, e.g. texts of religious nature, letters, temples oaths, or other private documents. Unidentified texts occupy the fifth and final section of the first part. The second part of the study, which comprises texts that originate from different Upper Egyptian localities, includes three sections, i.e. receipts, accounts, and lists of names.
Eine Dysbalance zwischen regulatorischen und proinflammatorischen T-Helferzellen kann zu Autoimmunerkrankungen führen. In dieser methodischen Arbeit wurde die Polarisierbarkeit von peripheren T-Lymphozyten durch verschiedene Zytokin-Stimuli untersucht. Hauptziel war es, CD4+CD25-CD127- Lymphozyten durch Stimulation mit einem IL-2 und TGFβ-beinhaltenden Zytokin-Cocktail (Treg-Cocktail) zu iTregs zu polarisieren und deren Suppressionsfunktion auf autologe Effektor-Leukozyten zu untersuchen.
Es erfolgte eine Phänotypisierung der PBMCs gesunder Probanden, insbesondere im Hinblick auf die Verteilung der T-Lymphozyten-Subpopulationen, deren Zytokinproduktion und FoxP3-Expression. Zudem wurden aus den PBMCs der Probanden Tregs (CD4+CD25+CD127low/-) sowie CD4+CD25-CD127- Zellen isoliert und deren Funktionsfähigkeit durch die Untersuchung ihrer Suppressionsfunktion auf autologe Effektor-Lymphozyten analysiert. Die Zellen wurden mittels verschiedener Zytokin-Cocktails in Richtung Treg sowie in Richtung Th17-Zellen polarisiert; anschließend wurde die Funktionsfähigkeit der polarisierten Zellen in Suppression-Assays gemessen.
Wir konnten zeigen, dass die CD4+CD25+CD127low/- Zellen Tregs mit der Fähigkeit zur Suppression der Proliferation autologer Effektor-Lymphozyten waren. Bei den CD4+CD25-CD127-Zellen handelte es sich um T-Lymphozyten ohne Suppressionsfunktion. Nach Stimulation der CD4+CD25-CD127-Zellen mit dem Treg-Cocktail zeigten die Zellen eine mit den Tregs vergleichbare Suppressionsfunktion.
Mit dieser Studie haben wir eine aktuelle methodische Quelle für die Untersuchung von Phänotyp und Funktion regulatorischer T-Zellen sowie für die Stimulation peripherer T-Lymphozyten hin zu Tregs geschaffen, die als Basis für Folgeversuche dienen soll, in denen Zellen von Patienten mit Autoimmunkrankheiten untersucht werden sollen. Da sich die Inflammation bei Autoimmunerkrankungen insbesondere in den betroffenen Geweben abspielt, wäre eine Studie anzustreben, in der aus dem Blut isolierte T-Lymphozyten den Zellen aus den entzündeten Geweben gegenübergestellt werden. Ergänzend sollte eine Phänotypisierung der Tregs und der CD4+CD25-CD127- Zellen nach der Zytokin-Stimulation erfolgen.
Zusammenfassend konnte die Plastizität peripherer T-Lymphozyten in Richtung Treg gezeigt werden. Besonders hervorzuheben ist die bislang wenig untersuchte Zellpopulation der CD4+CD25-CD127- Zellen, die eine vielversprechende Zellpopulation für die in vitro Induktion von Tregs darstellt.
Gold nanoparticles of diameter ca. 60 nm have been synthesized based on Turkevich and Frens protocols. We have demonstrated that the carboxyl-modified gold nanoparticles can be coupled covalently with antibodies (Ab) of interest using the EDC/NHS coupling procedure. Binding studies with Ab-grafted AuNPs and GpL fusion proteins proved that conjugation of AuNPs with antibodies enables immobilization of antibodies with preservation of a significant antigen binding capacity. More importantly, our findings showed that the conjugation of types of anti-TNF receptors antibodies such as anti-Fn14 antibodies (PDL192 and 5B6) (Aido et al., 2021), anti-CD40, anti-4-1BB and anti-TNFR2 with gold nanoparticles confers them with potent agonism. Thus, our results suggest that AuNPs can be utilized as a platform to immobilize anti-TNFR antibodies which, on the one hand, helps to enhance their agonistic activity in comparison to “free” inactive antibodies by mimicking the effect of cell-anchored antibodies or membrane-bound TNF ligands and, on the other hand, allows to develop new generations of drug delivery systems. These constructs are characterized with their biocompatibility and their tunable synthesis process.
In a further work part, we combined the benefits of the established system of Ab-AuNPs with materials used widely in the modern biofabrication approaches such as the photo-crosslinked hydrogels, methacrylate-modified gelatin (GelMA), combined with embedded variants of human cell lines. The acquired results demonstrated clearly that the attaching of proteins like antibodies to gold nanoparticles might reduce their release rate from the crosslinked hydrogels upon the very low diffusion of gold nanoparticles from the solid constructs to the surrounding medium yielding long-term local functioning proteins-attached particles. Moreover, our finding suggests that hydrogel-embedded AuNP-immobilized antibodies, e.g. anti-TNFα-AuNPs or anti-IL1-AuNPs enable local inhibitory functions,
To sum up, our results demonstrate that AuNPs can act as a platform to attach anti-TNFR antibodies to enhance their agonistic activity by resembling the output of cell-anchoring or membrane bounding. Gold nanoparticles are considered, thus, as promising tool to develop the next generation of drug delivery systems, which may contribute to cancer therapy. On top of that, the embedding of anti-inflammatory-AuNPs in the biofabricated hydrogel presents new innovative strategy of the treatment of autoinflammatory diseases.
Arrhythmogene Kardiomyopathie (ACM) ist eine genetische Herzerkrankung, die durch Herzinsuffizienz, ventrikuläre Arrhythmien und plötzlichen Herztod gekennzeichnet ist. Mutationen in desmosomalen Proteinen der Zelladhäsion, wie Plakophilin 2 (PKP2) und Plakoglobin (PG), sind die häufigste Ursache der familiären ACM. Wie gestörte Zelladhäsion zum ACM-Phänotyp führt, ist jedoch nur teilweise geklärt. Potentielle Mechanismen sind eine gestörte Kalzium-(Ca2+)-Homöostase, mitochondrialer oxidativer Stress und metabolische Störungen. Ziel dieser Studie ist es, die mitochondriale Energetik und die Ca2+ -Homöostase in kardio-restriktiven PKP2-Knockout-Mäusen (KO) im Alter von 4, 8 und 12 Wochen sowie in PG-Knockout- Mäusen im Alter von 6 Wochen zu untersuchen. Vier Wochen alte PKP2-KO-Mäuse zeigten frühe Anzeichen von ACM, während alle anderen Altersgruppen typische Kennzeichen von ACM rekapitulierten. Kontraktilität, die damit verbundenen Ca2+ - Transienten, der Redoxstatus und das mitochondriale Membranpotenzial (ΔΨm) isolierter Kardiomyozyten wurden mit einem IonOptix-System bei elektrischer und β- adrenerger Stimulation untersucht. Alle desmosomalen KO-Kardiomyozyten zeigten eine verringerte diastolische Sarkomerlänge, was auf eine diastolische Dysfunktion hinwies. In allen PKP2 KO Kardiomyozyten lag außerdem ein erhöhter intrazellulärer Ca2+ -Spiegel vor, während in den PG KO-Kardiomyozyten das intrazellulärer Ca2+ unverändert war. PKP2 KO- und PG KO-Kardiomyozyten wiesen keine Ca2+ - Sensibilisierung der Myofilamente auf. Zur weiteren Bewertung der mitochondrialen Funktion wurde eine hochauflösende Respirometrie in isolierten Herzmitochondrien bei gleichzeitiger Überwachung von ΔΨm in PKP2 KO und PG KO Mäusen durchgeführt, welche in allen Versuchs- und Kontrollgruppen vergleichbar war. Im Verlauf der Versuche blieb der Redoxstatus stabil und es konnte kein Exzess reaktiver Sauerstoffspezies (ROS) festgestellt werden. Daraus konnte gefolgert werden, dass weder PKP2 KO noch PG KO-Mäuse eine beeinträchtigte mitochondriale Atmung aufwiesen. Diese Studie zeigt, dass isolierte PKP2 KO- oder PG KO-Kardiomyozyten EC-Kopplungsdefekte ohne mitochondriale Dysfunktion aufwiesen. Eine mitochondriale Dysfunktion konnte als treibender Faktor für die Progression des ACM- Phänotyps in den vorgestellten Mausmodellen ausgeschlossen werden. Weitere Studien sind erforderlich, um die mitochondriale Funktion im Zusammenhang mit ACM zu entschlüsseln.
Introduction.
Mobile health (mHealth) integrates mobile devices into healthcare, enabling remote monitoring, data collection, and personalized interventions. Machine Learning (ML), a subfield of Artificial Intelligence (AI), can use mHealth data to confirm or extend domain knowledge by finding associations within the data, i.e., with the goal of improving healthcare decisions. In this work, two data collection techniques were used for mHealth data fed into ML systems: Mobile Crowdsensing (MCS), which is a collaborative data gathering approach, and Ecological Momentary Assessments (EMA), which capture real-time individual experiences within the individual’s common environments using questionnaires and sensors. We collected EMA and MCS data on tinnitus and COVID-19. About 15 % of the world’s population suffers from tinnitus.
Materials & Methods.
This thesis investigates the challenges of ML systems when using MCS and EMA data. It asks: How can ML confirm or broad domain knowledge? Domain knowledge refers to expertise and understanding in a specific field, gained through experience and education. Are ML systems always superior to simple heuristics and if yes, how can one reach explainable AI (XAI) in the presence of mHealth data? An XAI method enables a human to understand why a model makes certain predictions. Finally, which guidelines can be beneficial for the use of ML within the mHealth domain? In tinnitus research, ML discerns gender, temperature, and season-related variations among patients. In the realm of COVID-19, we collaboratively designed a COVID-19 check app for public education, incorporating EMA data to offer informative feedback on COVID-19-related matters. This thesis uses seven EMA datasets with more than 250,000 assessments. Our analyses revealed a set of challenges: App user over-representation, time gaps, identity ambiguity, and operating system specific rounding errors, among others. Our systematic review of 450 medical studies assessed prior utilization of XAI methods.
Results.
ML models predict gender and tinnitus perception, validating gender-linked tinnitus disparities. Using season and temperature to predict tinnitus shows the association of these variables with tinnitus. Multiple assessments of one app user can constitute a group. Neglecting these groups in data sets leads to model overfitting. In select instances, heuristics outperform ML models, highlighting the need for domain expert consultation to unveil hidden groups or find simple heuristics.
Conclusion.
This thesis suggests guidelines for mHealth related data analyses and improves estimates for ML performance. Close communication with medical domain experts to identify latent user subsets and incremental benefits of ML is essential.
Within this thesis, three main approaches for the assessment and investigation of altered hemodynamics like wall shear stress, oscillatory shear index and the arterial pulse wave velocity in atherosclerosis development and progression were conducted:
1. The establishment of a fast method for the simultaneous assessment of 3D WSS and PWV in the complete murine aortic arch via high-resolution 4D-flow MRI
2. The utilization of serial in vivo measurements in atherosclerotic mouse models using high-resolution 4D-flow MRI, which were divided into studies describing altered hemodynamics in late and early atherosclerosis
3. The development of tissue-engineered artery models for the controllable application and variation of hemodynamic and biologic parameters, divided in native artery models and biofabricated artery models, aiming for the investigation of the relationship between atherogenesis and hemodynamics
Chapter 2 describes the establishment of a method for the simultaneous measurement of 3D WSS and PWV in the murine aortic arch at, using ultra high-field MRI at 17.6T [16], based on the previously published method for fast, self-navigated wall shear stress measurements in the murine aortic arch using radial 4D-phase contrast MRI at 17.6 T [4]. This work is based on the collective work of Dr. Patrick Winter, who developed the method and the author of this thesis, Kristina Andelovic, who performed the experiments and statistical analyses. As the method described in this chapter is basis for the following in vivo studies and undividable into the sub-parts of the contributors without losing important information, this chapter was not split into the single parts to provide fundamental information about the measurement and analysis methods and therefore better understandability for the following studies. The main challenge in this chapter was to overcome the issue of the need for a high spatial resolution to determine the velocity gradients at the vascular wall for the WSS quantification and a high temporal resolution for the assessment of the PWV without prolonging the acquisition time due to the need for two separate measurements. Moreover, for a full coverage of the hemodynamics in the murine aortic arch, a 3D measurement is needed, which was achieved by utilization of retrospective navigation and radial trajectories, enabling a highly flexible reconstruction framework to either reconstruct images at lower spatial resolution and higher frame rates for the acquisition of the PWV or higher spatial resolution and lower frame rates for the acquisition of the 3D WSS in a reasonable measurement time of only 35 minutes. This enabled the in vivo assessment of all relevant hemodynamic parameters related to atherosclerosis development and progression in one experimental session. This method was validated in healthy wild type and atherosclerotic Apoe-/- mice, indicating no differences in robustness between pathological and healthy mice.
The heterogeneous distribution of plaque development and arterial stiffening in atherosclerosis [10, 12], however, points out the importance of local PWV measurements. Therefore, future studies should focus on the 3D acquisition of the local PWV in the murine aortic arch based on the presented method, in order to enable spatially resolved correlations of local arterial stiffness with other hemodynamic parameters and plaque composition.
In Chapter 3, the previously established methods were used for the investigation of changing aortic hemodynamics during ageing and atherosclerosis in healthy wild type and atherosclerotic Apoe-/- mice using the previously established methods [4, 16] based on high-resolution 4D-flow MRI. In this work, serial measurements of healthy and atherosclerotic mice were conducted to track all changes in hemodynamics in the complete aortic arch over time. Moreover, spatially resolved 2D projection maps of WSS and OSI of the complete aortic arch were generated. This important feature allowed for the pixel-wise statistical analysis of inter- and intragroup hemodynamic changes over time and most importantly – at a glance. The study revealed converse differences of local hemodynamic profiles in healthy WT and atherosclerotic Apoe−/− mice, with decreasing longWSS and increasing OSI, while showing constant PWV in healthy mice and increasing longWSS and decreasing OSI, while showing increased PWV in diseased mice. Moreover, spatially resolved correlations between WSS, PWV, plaque and vessel wall characteristics were enabled, giving detailed insights into coherences between hemodynamics and plaque composition. Here, the circWSS was identified as a potential marker of plaque size and composition in advanced atherosclerosis. Moreover, correlations with PWV values identified the maximum radStrain could serve as a potential marker for vascular elasticity. This study demonstrated the feasibility and utility of high-resolution 4D flow MRI to spatially resolve, visualize and analyze statistical differences in all relevant hemodynamic parameters over time and between healthy and diseased mice, which could significantly improve our understanding of plaque progression towards vulnerability. In future studies the relation of vascular elasticity and radial strain should be further investigated and validated with local PWV measurements and CFD.
Moreover, the 2D histological datasets were not reflecting the 3D properties and regional characteristics of the atherosclerotic plaques. Therefore, future studies will include 3D plaque volume and composition analysis like morphological measurements with MRI or light-sheet microscopy to further improve the analysis of the relationship between hemodynamics and atherosclerosis.
Chapter 4 aimed at the description and investigation of hemodynamics in early stages of atherosclerosis. Moreover, this study included measurements of hemodynamics at baseline levels in healthy WT and atherosclerotic mouse models. Due to the lack of hemodynamic-related studies in Ldlr-/- mice, which are the most used mouse models in atherosclerosis research together with the Apoe-/- mouse model, this model was included in this study to describe changing hemodynamics in the aortic arch at baseline levels and during early atherosclerosis development and progression for the first time. In this study, distinct differences in aortic geometries of these mouse models at baseline levels were described for the first time, which result in significantly different flow- and WSS profiles in the Ldlr-/- mouse model. Further basal characterization of different parameters revealed only characteristic differences in lipid profiles, proving that the geometry is highly influencing the local WSS in these models. Most interestingly, calculation of the atherogenic index of plasma revealed a significantly higher risk in Ldlr-/- mice with ongoing atherosclerosis development, but significantly greater plaque areas in the aortic arch of Apoe-/- mice. Due to the given basal WSS and OSI profile in these two mouse models – two parameters highly influencing plaque development and progression – there is evidence that the regional plaque development differs between these mouse models during very early atherogenesis.
Therefore, future studies should focus on the spatiotemporal evaluation of plaque development and composition in the three defined aortic regions using morphological measurements with MRI or 3D histological analyses like LSFM. Moreover, this study offers an excellent basis for future studies incorporating CFD simulations, analyzing the different measured parameter combinations (e.g., aortic geometry of the Ldlr-/- mouse with the lipid profile of the Apoe-/- mouse), simulating the resulting plaque development and composition. This could help to understand the complex interplay between altered hemodynamics, serum lipids and atherosclerosis and significantly improve our basic understanding of key factors initiating atherosclerosis development.
Chapter 5 describes the establishment of a tissue-engineered artery model, which is based on native, decellularized porcine carotid artery scaffolds, cultured in a MRI-suitable bioreactor-system [23] for the investigation of hemodynamic-related atherosclerosis development in a controllable manner, using the previously established methods for WSS and PWV assessment [4, 16]. This in vitro artery model aimed for the reduction of animal experiments, while simultaneously offering a simplified, but completely controllable physical and biological environment. For this, a very fast and gentle decellularization protocol was established in a first step, which resulted in porcine carotid artery scaffolds showing complete acellularity while maintaining the extracellular matrix composition, overall ultrastructure and mechanical strength of native arteries. Moreover, a good cellular adhesion and proliferation was achieved, which was evaluated with isolated human blood outgrowth endothelial cells. Most importantly, an MRI-suitable artery chamber was designed for the simultaneous cultivation and assessment of high-resolution 4D hemodynamics in the described artery models. Using high-resolution 4D-flow MRI, the bioreactor system was proven to be suitable to quantify the volume flow, the two components of the WSS and the radStrain as well as the PWV in artery models, with obtained values being comparable to values found in literature for in vivo measurements. Moreover, the identification of first atherosclerotic processes like intimal thickening is achievable by three-dimensional assessment of the vessel wall morphology in the in vitro models. However, one limitation is the lack of a medial smooth muscle cell layer due to the dense ECM. Here, the utilization of the laser-cutting technology for the generation of holes and / or pits on a microscale, eventually enabling seeding of the media with SMCs showed promising results in a first try and should be further investigated in future studies. Therefore, the proposed artery model possesses all relevant components for the extension to an atherosclerosis model which may pave the way towards a significant improvement of our understanding of the key mechanisms in atherogenesis.
Chapter 6 describes the development of an easy-to-prepare, low cost and fully customizable artery model based on biomaterials. Here, thermoresponsive sacrificial scaffolds, processed with the technique of MEW were used for the creation of variable, biomimetic shapes to mimic the geometric properties of the aortic arch, consisting of both, bifurcations and curvatures. After embedding the sacrificial scaffold into a gelatin-hydrogel containing SMCs, it was crosslinked with bacterial transglutaminase before dissolution and flushing of the sacrificial scaffold. The hereby generated channel was subsequently seeded with ECs, resulting in an easy-to-prepare, fast and low-cost artery model. In contrast to the native artery model, this model is therefore more variable in size and shape and offers the possibility to include smooth muscle cells from the beginning. Moreover, a custom-built and highly adaptable perfusion chamber was designed specifically for the scaffold structure, which enabled a one-step creation and simultaneously offering the possibility for dynamic cultivation of the artery models, making it an excellent basis for the development of in vitro disease test systems for e.g., flow-related atherosclerosis research. Due to time constraints, the extension to an atherosclerosis model could not be achieved within the scope of this thesis. Therefore, future studies will focus on the development and validation of an in vitro atherosclerosis model based on the proposed bi- and three-layered artery models.
In conclusion, this thesis paved the way for a fast acquisition and detailed analyses of changing hemodynamics during atherosclerosis development and progression, including spatially resolved analyses of all relevant hemodynamic parameters over time and in between different groups. Moreover, to reduce animal experiments, while gaining control over various parameters influencing atherosclerosis development, promising artery models were established, which have the potential to serve as a new platform for basic atherosclerosis research.
In einer ethnologisch-vergleichenden Perspektive zeigt Christoph Antweiler auf, dass auch in nichtindustriellen Gesellschaften Gewaltlosigkeit aktiv hergestellt werden muss, welche Ressourcen und kulturellen Kompetenzen dazu erforderlich sind und welche Rolle dritte Personen oder Instanzen dabei spielen können. Mechanismen gewaltfreier Konfliktregelung beschreibt er anhand des Fallbeispiels der Semai in Malaysia.
Die vorliegende Arbeit befasst sich mit der Beschreibung des Status quo der Versorgungsrealität von BARMER Patient*innen, welche nach operativem inguinalen- oder femoralen Hernienverschluss an Schmerzen litten und geht in weiterer Folge dessen Hinweisen auf CPIP nach. Es fand die Sekundärdatenanlyse von Routinedaten der BARMER Krankenkasse Anwendung. Die Stichprobe umfasste 11221 Patient*innen, von denen 77.7% unter keinen Leistenschmerzen im prä- oder postoperativen Zusammenhang mit dem Eingriff litten, bezeichnet als Gruppe „Pain 0“. 4.2% litten sowohl innerhalb von 365 Tagen vor- als auch nach dem Krankenhausaufenthalt an Schmerzen, was als chronisch zu bezeichnen war und unter Gruppe „Pain 2“ geführt wurde. 8.5% der Patient*innen litten nur innerhalb von 365 Tagen nach Entlassung an Schmerzen, was nur im erweiterten Sinne auf CPIP hinwies, da der Ausschluss der ersten 90 Tage postoperativ nicht in der Definition der Gruppe enthalten war. Diese Patient*innen gehörten der Gruppe „Pain 1“ an. Die Gruppe „Pain 3“ umfasste diejenigen 9.6% der Patient*innen, welche innerhalb von 365 Tagen präoperativ an Schmerzen litten. Obwohl keine postoperativen Leistenschmerzen für diese Patient*innen codiert worden sind, stellte sich eine bessere Versorgung als die der Gruppe „Pain 0“ dar.
Patient*innen der Gruppe „Pain 2“ mit der längsten Schmerzerfahrung wurden signifikant besser versorgt. Diese Gruppe, welche an chronischen, postoperativen, inguinalen Schmerzen litt, zeichnete sich durch eine signifikant jüngere Patient*innenklientel aus. Der Anteil an Frauen war signifikant höher. Begleitende psychiatrische Komorbiditäten traten signifikant häufiger auf. Die Versorgung dieser Patient*innengruppe war signifikant besser, allerdings vor allem hinsichtlich der psychologischen und psychiatrischen Betreuung nicht ausreichend gut. Die Mehrzahl der Analysen war hochsignifikant, deren Effektstärke fiel klein aus.
The variant surface glycoprotein (VSG) of African trypanosomes plays an essential role in protecting the parasites from host immune factors. These trypanosomes undergo antigenic variation resulting in the expression of a single VSG isoform out of a repertoire of around 2000 genes. The molecular mechanism central to the expression and regulation of the VSG is however not fully understood.
Gene expression in trypanosomes is unusual due to the absence of typical RNA polymerase II promoters and the polycistronic transcription of genes. The regulation of gene expression is therefore mainly post-transcriptional. Regulatory sequences, mostly present in the 3´ UTRs, often serve as key elements in the modulation of the levels of individual mRNAs. In T. brucei VSG genes, a 100 % conserved 16mer motif within the 3´ UTR has been shown to modulate the stability of VSG transcripts and hence their expression. As a stability-associated sequence element, the absence of nucleotide substitutions in the motif is however unusual. It was therefore hypothesised that the motif is involved in other essential roles/processes besides stability of the VSG transcripts.
In this study, it was demonstrated that the 100 % conservation of the 16mer motif is not essential for cell viability or for the maintenance of functional VSG protein levels. It was further shown that the intact motif in the active VSG 3´ UTR is neither required to promote VSG silencing during switching nor is it needed during differentiation from bloodstream forms to procyclic forms. Crosstalk between the VSG and procyclin genes during differentiation to the insect vector stage is also unaffected in cells with a mutated 16mer motif. Ectopic overexpression of a second VSG however requires the intact motif to trigger silencing and exchange of the active VSG, suggesting a role for the motif in transcriptional VSG switching. The 16mer motif therefore plays a dual role in VSG in situ switching and stability of VSG transcripts. The additional role of the 16mer in the essential process of antigenic variation appears to be the driving force for the 100 % conservation of this RNA motif.
A screen aimed at identifying candidate RNA-binding proteins interacting with the 16mer motif, led to the identification of a DExD/H box protein, Hel66. Although the protein did not appear to have a direct link to the 16mer regulation of VSG expression, the DExD/H family of proteins are important players in the process of ribosome biogenesis. This process is relatively understudied in trypanosomes and so this candidate was singled out for detailed characterisation, given that the 16mer story had reached a natural end point. Ribosome biogenesis is a major cellular process in eukaryotes involving ribosomal RNA, ribosomal proteins and several non-ribosomal trans-acting protein factors. The DExD/H box proteins are the most important trans-acting protein factors involved in the biosynthesis of ribosomes. Several DExD/H box proteins have been directly implicated in this process in yeast. In trypanosomes, very few of this family of proteins have been characterised and therefore little is known about the specific roles they play in RNA metabolism. Here, it was shown that Hel66 is involved in rRNA processing during ribosome biogenesis. Hel66 localises to the nucleolus and depleting the protein led to a severe growth defect. Loss of the protein also resulted in a reduced rate of global translation and accumulation of rRNA processing intermediates of both the small and large ribosomal subunits. Hel66 is therefore an essential nucleolar DExD/H protein involved in rRNA processing during ribosome biogenesis. As very few protein factors involved in the processing of rRNAs have been described in trypanosomes, this finding represents an important platform for future investigation of this topic.
Auswirkungen der Genpolymorphismen ASIC1, BDNF und NPSR1 auf die Antizipationsphase aversiver Reize

In dieser Arbeit wurden einerseits die Antizipationsphasen von aversiven gegenüber neutralen Reizen anhand von Messungen der Hautleitfähigkeit und der Startle-Reaktion untersucht. Andererseits wurde die Hautleitfähigkeit auch während der Präsentation aversiver und neutraler Reize mit dem Ziel gemessen, signifikante Unterschiede festzustellen. Insbesondere wurden die Auswirkungen der Allele der Gene ASIC1 und der Interaktion der Genallele BDNF und NPSR1 betrachtet.
Ziel der vorliegenden Arbeit war es, den Einfluss der Risikogene auf die physiologische Angstreaktion und die subjektive Angstwahrnehmung zu untersuchen. Hierzu wurden den genotypisierten Probanden aversive und neutrale Videos präsentiert. Vor jedem Video erfolgte die Ankündigung, ob es sich um ein neutrales oder aversives Video handelt, wodurch bei Letzterem im Allgemeinen antizipatorische Angst – Erwartungsangst – hervorgerufen wird.
Im Vergleich der Antizipationsphase vor Darbietung aversiver Videos mit der Antizipationsphase vor neutralen Videos konnte eine erhöhte Startle-Amplitude gemessen werden. Jedoch konnte weder anhand der Veränderung der Hautleitfähigkeit noch anhand der Startle-Amplitude ein signifikanter Unterschied bei Trägern und Nicht- Trägern der Risikogenallelen in der Antizipationsphase festgestellt werden. Während der Präsentation der Videos konnte für die aversiven Videos im Vergleich zu den neutralen eine erhöhte Hautleitfähigkeit gemessen werden. Ebenfalls konnte bei der Darbietung von aversiven Videos bei den Trägern der Genallel-Interaktion NPSR1 AT/TT * BDNF GG und den Trägern des Risikogenallels ASIC1TT eine erhöhte Hautleitfähigkeit gemessen werden. So konnte mit den Ergebnissen dieser Arbeit belegt werden, dass Antizipationsangst auslösbar und anhand der Startle-Amplitude messbar ist.
Um Antizipationsangst festzustellen oder diese bei Risikogenallel-Träger zu untersuchen, waren die Ergebnisse bezüglich der Hautleitfähigkeit jedoch weniger aussagekräftig als erwartet.
Allgemein konnte die Interaktion NPSR1 AT/TT * BDNF GG und ASIC1 TT als Risikogenallele bezüglich einer verstärken Reaktion auf aversive Reize bestätigt werden. Weitere Studien sind notwendig, um die genetische Komponente von Angst und damit auch von Angsterkrankungen näher zu beleuchten, damit zukünftige Diagnostik- und Therapieansätze präzise entwickelt werden können.
Postoperative pulmonale Komplikationen (PPC) stellen den Hauptgrund für erhöhte Morbidität und Mortalität sowie eine längere stationäre Liegedauer nach chirurgischen Eingriffen dar. Die Elektrische Impedanztomographie (EIT) ermöglicht als strahlungsfreie Methode die bettseitige Visualisierung der regionalen pulmonalen Ventilation in einem thorakalen Querschnittsbereich über den zeitlichen Verlauf.
Die Hauptfragestellung dieser Studie war die perioperativen Veränderungen der regionalen pulmonalen Ventilation bei spontanatmenden Patienten nach abdominalchirurgischen Eingriffen in Allgemeinnarkose bis in die späte postoperative Phase zu untersuchen. Zusätzlich untersuchten wir die Lungenfunktion mittels Spirometrie. Wir nahmen eine Verschiebung der pulmonalen Ventilation in dorso-ventraler Richtung an, sowie eine postoperativ reduzierte Vitalkapazität, z.B. durch Atelektasen oder Pleuraergüsse.
In die prospektive Observationsstudie wurden 36 erwachsene Patienten eingeschlossen, die sich einem elektiven abdominalchirurgischen Eingriff unter Allgemeinanästhesie unterzogen und ein mittleres Risiko gemäß ARISCAT Score für die Entwicklung von PPC aufwiesen. Präoperativ, sowie am 1. und 3. postoperativen Tag erfolgte die Untersuchung der pulmonalen Ventilation mittels EIT in Spontanatmung, Errechnung des Center of Ventilation (COV), sowie eine Lungenfunktionsprüfung mittels Spirometrie.
Nach abdominalchirurgischen Operationen kam es zu einer statistisch signifikanten und bis zum 3. postoperativen Tag anhaltenden Verschiebung der pulmonalen Ventilation nach ventral (COVy präop. 16,5; 1. Tag postop. 17,8; 3. Tag postop. 17,4). Zudem zeigte sich eine anhaltend reduzierte Forcierten Vitalkapazität in % vom Sollwert (FVC%Soll): präop. 93%; 1. Tag postop. 58%; 3. Tag postop. 64%. Am 3. postoperativen Tag bestand unter forcierter Atmung eine negative Assoziation zwischen der Änderung des COVy und der Änderung der FVC%Soll. PPC traten bei 10 Patienten in Form von respiratorischer Insuffizienz, Atelektase und Pleuraerguss auf. Bei diesen Patienten zeigte die EIT keine komplikationsspezifischen Bilder.
Abdominalchirurgische Operationen hatten hat einen relevanten Einfluss auf die postoperative regionale Lungenventilation und somit auf die Entstehung von PPC. Die EIT hilft die Entstehung von PPC besser zu verstehen und Strategien zur Vermeidung solcher im klinischen Alltag zu implementieren.
This doctoral thesis investigates magneto-optical properties of mercury telluride layers grown tensile strained on cadmium telluride substrates. Here, layer thicknesses start above the usual quantum well thickness of about 20 nm and have a upper boundary around 100 nm due to lattice relaxation effects. This kind of layer system has been attributed to the material class of three-dimensional topological insulators in numerous publications. This class stands out due to intrinsic boundary states which cross the energetic band gap of the layer's bulk.
In order to investigate the band structure properties in a narrow region around the Fermi edge, including possible boundary states, the method of highly precise time-domain Terahertz polarimetry is used. In the beginning, the state of the art of Teraherz technology at the start of this project is discussed, moving on to a detailed description and characterization of the self-built measurement setup. Typical standard deviation of a polarization rotation or ellipticity measurement are on the order of 10 to 100 millidegrees, according to the transmission strength through investigated samples. A range of polarization spectra, depending on external magnetic fields up to 10 Tesla, can be extracted from the time-domain signal via Fourier transformation.
The identification of the actual band structure is done by modeling possible band structures by means of the envelope function approximation within the framework of the k·p method. First the bands are calculated based on well-established model parameters and from them the possible optical transitions and expected ellipticity spectra, all depending on external magnetic fields and the layer's charge carrier concentration. By comparing expected with measured spectra, the validity of k·p models with varying depths of detail is analyzed throughout this thesis. The rich information encoded in the ellipitcity spectra delivers key information for the attribution of single optical transitions, which are not part of pure absorption spectroscopy. For example, the sign of the ellipticity signals is linked to the mix of Landau levels which contribute to an optical transition, which shows direct evidence for bulk inversion asymmetry effects in the measured spectra.
Throughout the thesis, the results are compared repeatedly with existing publications on the topic. It is shown that the models used there are often insufficient or, in worst case, plainly incorrect. Wherever meaningful and possible without greater detours, the differences to the conclusions that can be drawn from the k·p model are discussed.
The analysis ends with a detailed look on remaining differences between model and measurement. It contains the quality of model parameters as well as different approaches to integrate electrostatic potentials that exist in the structures into the model.
An outlook on possible future developments of the mercury cadmium telluride layer systems, as well as the application of the methods shown here onto further research questions concludes the thesis.
Development Of A Human iPSC-Derived Cortical Neuron Model Of Adaptor- Protein-Complex-4-Deficiency

Adaptor-protein-4-deficiency (AP-4-deficiency) is an autosomal-recessive childhood- onset form of complicated hereditary spastic paraplegia (HSP) caused by bi-allelic loss- of-function mutations in one of the four subunits of the AP-4-complex. These four conditions are named SPG47 (AP4B1, OMIM #614066), SPG50 (AP4M1, OMIM #612936), SPG51 (AP4E1, OMIM #613744) and SPG52 (AP4S1, OMIM #614067), respectively and all present with global developmental delay, progressive spasticity and seizures. Imaging features include a thinning of the corpus callosum, ventriculomegaly and white matter changes. AP-4 is a highly conserved heterotetrameric complex, which is responsible for polarized sorting of transmembrane cargo including the autophagy- related protein 9 A (ATG9A). Loss of any of the four subunits leads to an instable complex and defective sorting of AP-4-cargo. ATG9A is implicated in autophagosome formation and neurite outgrowth. It is missorted in AP-4-deficient cells and CNS-specific knockout of Atg9a in mice results in a phenotype reminiscent of AP-4-deficiency. However, the AP-4-related cellular phenotypes including ATG9A missorting have not been investigated in human neurons.
Thus, the aim of this study is to provide the first human induced pluripotent stem cell- derived (iPSC) cortical neuron model of AP-4-deficiency to explore AP-4-related phenotypes in preparation for a high-content screening. Under the hypothesis that AP-4- deficiency leads to ATG9A missorting, elevated ATG9A levels, impaired autophagy and neurite outgrowth in human iPSC-derived cortical neurons, in vitro biochemical and imaging assays including automated high-content imaging and analysis were applied. First, these phenotypes were investigated in fibroblasts from three patients with compound heterozygous mutations in the AP4B1 gene and their sex-matched parental controls. The same cell lines were used to generate iPSCs and differentiate them into human excitatory cortical neurons.
This work shows that ATG9A is accumulating in the trans-Golgi-network in AP-4- deficient human fibroblasts and that ATG9A levels are increased compared to parental controls and wild type cells suggesting a compensatory mechanism. Protein levels of the AP4E1-subunit were used as a surrogate marker for the AP-4-complex and were decreased in AP-4-deficient fibroblasts with co-immunoprecipitation confirming the instability of the complex. Lentiviral re-expression of the AP4B1-subunit rescues this corroborating the fact that a stable AP-4-complex is needed for ATG9A trafficking. Surprisingly, autophagic flux was present in AP-4-deficient fibroblasts under nutrient- rich and starvation conditions. These phenotypic markers were evaluated in iPSC-derived cortical neurons and here, a robust accumulation of ATG9A in the juxtanuclear area was seen together with elevated ATG9A protein levels. Strikingly, assessment of autophagy markers under nutrient-rich conditions showed alterations in AP-4-deficient iPSC- derived cortical neurons indicating dysfunctional autophagosome formation. These findings point towards a neuron-specific impairment of autophagy and need further investigation. Adding to the range of AP-4-related phenotypes, neurite outgrowth and branching are impaired in AP-4-deficient iPSC-derived cortical neurons as early as 24h after plating and together with recent studies point towards a distinct role of ATG9A in neurodevelopment independent of autophagy.
Together, this work provides the first patient-derived neuron model of AP-4-deficiency and shows that ATG9A is sorted in an AP-4-dependent manner. It establishes ATG9A- related phenotypes and impaired neurite outgrowth as robust markers for a high-content screening. This disease model holds the promise of providing a platform to further study AP-4-deficiency and to search for novel therapeutic targets.
Spielfilme gelten im Sinne einer „Visual History“ als wertvolle medizinhistorische Quellen. Dass die Arztfilme der DDR-Zeit ebenfalls als solche zu betrachten sind, da sie realhistorische Parallelen aufweisen, soll dieses Projekt zeigen. Anhand dreier Spielfilme aus den verschiedenen Jahrzehnten, in denen die DDR Bestand hatte, werden für die damalige Zeit typische Konflikte und Themen des Arztseins in der DDR näher beleuchtet. Die drei Hauptfilme dieses Projekts – „Ärzte“ (1960), „Dr. med. Sommer II“ (1970) und „Ärztinnen“ (1983/84) – wurden hinsichtlich ihrer Hauptfiguren, Filmtechnik und -musik analysiert und mittels Filmkritiken, Werbematerial und Aufsätzen aus der damaligen Zeit in einen realhistorischen Kontext gesetzt. Außerdem wurden zur besseren filmgeschichtlichen Einordnung weitere Arztfilme aus der DDR in die Arbeit miteinbezogen. Das Medium Film spielte in Zeiten der DDR auch zur allgemeinen gesellschaftlichen Beeinflussung eine wichtige Rolle. Durch die Analyse der Filme unter Einbeziehung von historischen Zeitungsartikeln und Werbematerial wird das Bild eines sozialistischen Idealmenschen und -arztes, wie von der SED propagiert, dargestellt und untersucht.
Articular cartilage defects represent one of the most challenging clinical problem for orthopedic surgeons and cartilage damage after trauma can result in debilitating joint pain, functional impairment and in the long-term development of osteoarthritis. The lateral cartilage-cartilage integration is crucial for the long-term success and to prevent further tissue degeneration. Tissue adhesives and sealants are becoming increasingly more popular and can be a beneficial approach in fostering tissue integration, particularly in tissues like cartilage where alternative techniques, such as suturing, would instead introduce further damage. However, adhesive materials still require optimization regarding the maximization of adhesion strength on the one hand and long-term tissue integration on the other hand. In vitro models can be a valuable support in the investigation of potential candidates and their functional mechanisms. For the conducted experiments within this work, an in vitro disc/ring model obtained from porcine articular cartilage tissue was established. In addition to qualitative evaluation of regeneration, this model facilitates the implementation of biomechanical tests to quantify cartilage integration strength. Construct harvesting for histology and other evaluation methods could be standardized and is ethically less questionable compared to in vivo testing. The opportunity of cell culture technique application for the in vitro model allowed a better understanding of cartilage integration processes.
Tissue bonding requires chemical or physical interaction of the adhesive material and the substrate. Adhesive hydrogels can bind to the defect interface and simultaneously fill the gap of irregularly shaped defect voids. Fibrin gels are derived from the physiological blood-clot formation and are clinically applied for wound closure. Within this work, comparisons of different fibrin glue formulations with the commercial BioGlue® were assessed, which highlighted the need for good biocompatibility when applied on cartilage tissue in order to achieve satisfying long-term integration. Fibrin gel formulations can be adapted with regard to their long-term stability and when applied on cartilage disc/ring constructs improved integrative repair is observable. The kinetic of repairing processes was investigated in fibrin-treated cartilage composites as part of this work. After three days in vitro cultivation, deposited extracellular matrix (ECM) was obvious at the glued interface that increased further over time. Interfacial cell invasion from the surrounding native cartilage was detected from day ten of tissue culture. The ECM formation relies on molecular factors, e.g., as was shown representatively for ascorbic acid, and contributes to increasing integration strengths over time. The experiments performed with fibrin revealed that the treatment with a biocompatible adhesive that allows cartilage neosynthesis favors lateral cartilage integration in the long term. However, fibrin has limited immediate bonding strength, which is disadvantageous for use on articular cartilage that is subject to high mechanical stress. The continuing aim of this thesis was to further develop adhesive mechanisms and new adhesive hydrogels that retain the positive properties of fibrin but have an increased immediate bonding strength.
Two different photochemical approaches with the advantage of on-demand bonding were tested. Such treatment potentially eases the application for the professional user. First, an UV light induced crosslinking mechanism was transferred to fibrin glue to provide additional bonding strength. For this, the cartilage surface was functionalized with highly reactive light-sensitive diazirine groups, which allowed additional covalent bonds to the fibrin matrix and thus increased the adhesive strength. However, the disadvantages of this approach were the multi-step bonding reactions, the need for enzymatic pretreatment of the cartilage, expensive reagents, potential UV-light damage, and potential toxicity hazards. Due to the mentioned disadvantages, no further experiments, including long-term culture, were carried out. A second photosensitive approach focused on blue light induced crosslinking of fibrinogen (RuFib) via a photoinitiator molecule instead of using thrombin as a crosslinking mediator like in normal fibrin glue. The used ruthenium complex allowed inter- and intramolecular dityrosine binding of fibrinogen molecules. The advantage of this method is a one-step curing of fibrinogen via visible light that further achieved higher adhesive strengths than fibrin. In contrast to diazirine functionalization of cartilage, the ruthenium complex is of less toxicological concern. However, after in vitro cultivation of the disc/ring constructs, there was a decrease in integration strength. Compared to fibrin, a reduced cartilage synthesis was observed at the defect. It is also disadvantageous that a direct adjustment of the adhesive can only be made via protein concentration, since fibrinogen is a natural protein that has a fixed number of tyrosine binding sites without chemical modification.
An additional cartilage adhesive was developed that is based on a mussel-inspired adhesive mechanism in which reactivity to a variety of substrates is enabled via free DOPA amino acids. DOPA-based adhesion is known to function in moist environments, a major advantage for application on water-rich cartilage tissue surrounded by synovial liquid. Reactive DOPA groups were synthetically attached to a polymer, here POx, to allow easy chemical modifiability, e.g. insertion of hydrolyzable ester motifs for tunable degradation. The possibility of preparing an adhesive hybrid hydrogel of POx in combination with fibrinogen led to good cell compatibility as was similarly observed with fibrin, but with increased immediate adhesive strength. Degradation could be adjusted by the amount of ester linkages on the POx and a direct influence of degradation rates on the development of integration in the in vitro model could be shown.
Hydrogels are well suited to fill defect gaps and immediate integration can be achieved via adhesive properties. The results obtained show that for the success of long-term integration, a good ability of the adhesive to take up synthesized ECM components and cells to enable regeneration is required. The degradation kinetics of the adhesive must match the remodeling process to avoid intermediate loss of integration power and to allow long-term firm adhesion to the native tissue.
Hydrogels are not only important as adhesives for smaller lesions, but also for filling large defect volumes and populating them with cells to produce tissue engineered cartilage. Many different hydrogel types suitable for cartilage synthesis are reported in the literature. A long-term stable fibrin formulation was tested in this work not only as an adhesive but also as a bulk hydrogel construct. Agarose is also a material widely used in cartilage tissue engineering that has shown good cartilage neosynthesis and was included in integration assessment. In addition, a synthetic hyaluronic acid-based hydrogel (HA SH/P(AGE/G)) was used. The disc/ring construct was adapted for such experiments and the inner lumen of the cartilage ring was filled with the respective hydrogel. In contrast to agarose, fibrin and HA-SH/P(AGE/G) gels have a crosslink mechanism that led to immediate bonding upon contact with cartilage during curing. The enhanced cartilage neosynthesis in agarose compared to the other hydrogel types resulted in improved integration during in vitro culture. This shows that for the long-term success of a treatment, remodeling of the hydrogel into functional cartilage tissue is a very high priority. In order to successfully treat larger cartilage defects with hydrogels, new materials with these properties in combination with chemical modifiability and a direct adhesion mechanism are one of the most promising approaches.
Zusammenfassend kann davon ausgegangen werden, dass die späte Rezeption Landstei-ners und seiner Entdeckung der Blutgruppen sowie die langsame Entwicklung der flä-chendeckenden Anwendung der Bluttransfusion als lebensrettende medizinische Maß-nahme auf mehreren Problematiken beruhte. Diese sollen hier nochmal einzeln kurz be-leuchtet werden.
Einer der Gründe für die verspätete Rezeption kann sicherlich in Karl Landsteiners zu-rückhaltenden und etwas sperrigen Persönlichkeit gesehen werden, aufgrund derer die Gruppe seiner Anhänger in Österreich eher klein blieb. Nur wenige kamen mit seiner Ar-beitsweise und dem Klima, das in seinem Laboratorium vorherrschte, zurecht. Es dauerte eine Weile, bis der Großteil der Wissenschaftler über Landsteiners komplexe Persönlich-keitsstruktur, mit der er vielfach aneckte, hinwegsahen und ihn für den genialen Forscher-geist feierten, den er besaß. Seine Scheu vor Aufmerksamkeit führte dazu, dass er seine Entdeckung der Blutgruppen nicht aktiv und nachdrücklich publik zu machen versuchte. Er war zwar ein herausragender Wissenschaftler, strebte aber nicht nach einem Leben im Rampenlicht. Hätte er das gewollt, wäre seine Entdeckung mutmaßlich nicht nur in einem einzigen Artikel von ihm publiziert worden, sondern Landsteiner hätte daraufhin weiter nachgeforscht und seine Ergebnisse noch mehr mit der Entwicklung der Transfusionsme-dizin in Verbindung gestellt. Stattdessen wechselte er das Forschungsgebiet und beauf-tragte seine Assistenten, das Thema weiterzuverfolgen. Dies führte erst dazu, dass er mit der Entdeckung der vierten Blutgruppe schon gar nicht mehr in Verbindung gebracht wurde und öffnete schließlich in den Folgejahren anderen Medizinern das Tor zu weiterer Forschung auf diesem Gebiet. Es gab neue Blutgruppen-Nomenklaturen von verschiede-nen Wissenschaftlern und die Errungenschaften bei der Entwicklung der Bluttransfusion standen letztendlich scheinbar in keiner Verbindung mehr mit Karl Landsteiner.
Mit Sicherheit spielte jedoch Landsteiners Wahl bei der Entscheidung für sein Institut in Wien eine Rolle, durch die er in einen alten wissenschaftlichen Streit hineingezogen wur-de. Die Ursprünge des Konflikts zwischen Max Gruber und Paul Ehrlich hatten die Sci-entific Community in zwei Lager entzweit. Durch Landsteiners Tätigkeit am Hygiene-Institut und die Unterstützung Grubers, hatte er sich in Bezug auf die Aussicht auf wis-senschaftlichen Erfolg für die falsche Seite entschieden. Da es in Landsteiners Natur lag, nur wissenschaftlichen Tatsachen zu vertrauen und er sich nicht von Opportunitätsdenken abhalten ließ, die Wahrheit ans Licht zu bringen, stellte er durch seine Handlungen die gegenüberstehende Konfliktpartei bloß. Dass dies ausgerechnet die Gruppe um Paul Ehr-lich war, der den Rückhalt des Großteils der Wissenschaftsgesellschaft genoss und zudem das Gebiet der Immunologie, das Landsteiner erforschte, bis dahin beherrschte, machte es dem Blutgruppenentdecker fast unmöglich, in der österreichischen Universitätslandschaft Erfolge zu feiern.
Auch der Verdacht, Karl Landsteiners Erfolg sei von antisemitischen Vorurteilen beein-trächtigt worden, darf nicht vernachlässigt werden. Landsteiner fühlte sich sein Leben lang von seinen jüdischen Wurzeln regelrecht verfolgt, wie an dem Gerichtsprozess gegen die jüdische Enzyklopädie zu erkennen ist. Er schien sich geradezu dafür zu schämen, sonst hätte er wohl seinen Sohn in diesen Teil der Familiengeschichte eingeweiht. Ob es sich dabei eher um ein Selbstbewusstseinsproblem Landsteiners handelte, oder ob es hier-für triftige Gründe in Form von antisemitischen Anfeindungen während seiner Zeit in Wien gab, konnte aufgrund der mangelhaften Quellenlage nicht abschließend geklärt wer-den. Deutlich wird aber, dass der zunehmende Antisemitismus und der Mangel an Auf-stiegschancen in Wien ihn aus seiner Heimat vertrieben.
Der am stärksten zu gewichtende Grund für die verspätete Rezeption der Entdeckung der Blutgruppen und deren Bedeutung waren die Hemmungen vor der Anwendung aufgrund von etlichen ungelösten technischen Problemen. Hinzu kamen die Berichte über schwer-wiegenden Folgereaktionen auf Bluttransfusionen, die in der Zeit vor Entdeckung der Blutgruppen verzeichnet wurden, weshalb die breite Masse der Ärzteschaft sich von die-ser Technik abgewandt hatte. Es brauchte eine gewisse Zeit und eine Reihe vertrauens-würdiger Studien, aus denen hervorging, dass diese schweren Reaktionen eben genau auf dem fehlenden Wissen über die Blutgruppen beruhten, bis sich die breite Masse wieder an diese therapeutische Maßnahme herantraute. Da, wie heute bekannt ist, die Einteilung nach dem AB0-System noch nicht alle Gefahren der Bluttransfusion aus dem Weg ge-räumt hat, sondern weitere Faktoren wie der Rhesusfaktor und eventuelle unregelmäßige Blutgruppen eine Rolle spielten, kam es trotz Landsteiners bahnbrechender Entdeckung doch weiterhin zu Zwischenfällen, mit teilweise tödlichem Ausgang nach Bluttransfusio-nen nach dem AB0–System. Des Weiteren spielte das Fehlen einer einfachen, für den Arzt in der Praxis geeigneten Technik eine Rolle. Außer der, aufgrund von Landois Be-obachtungen, unpopulären Möglichkeit der Transfusion von defibriniertem Blut gab es mehr als ein Jahrzehnt nach Landsteiners Entdeckung nur die Möglichkeit der direkten Transfusion. Hierfür mussten sich nicht nur Spender und Empfänger im selben Raum befinden, sondern es bedurfte auch ausgezeichneter chirurgischer Fähigkeiten für eine Gefäßnaht oder benötigte später, nach Erfindung zahlreicher Apparaturen, zumindest viel Personal und Zeit für das aufwändige Verfahren. Diese Voraussetzungen waren aus-schließlich in großen Kliniken gegeben und auch hier nicht als Standardverfahren. Als 1914 die gerinnungshemmende Wirkung von Natriumcitrat entdeckt wurde, verhalf dies der Bluttransfusion in vielen Ländern zum Durchbruch. Die neu entdeckte Möglichkeit der Konservierung von Blut kam rechtzeitig zum Beginn des Ersten Weltkrieges, der, mit der Vielzahl an Verwundeten und der Notwendigkeit der Kompensation großer Blutver-luste, den medizinischen Fortschritt auf diesem Gebiet deutlich vorantrieb. Jedoch gab es auch hier noch viele Gegner, vor allem in Deutschland und Landsteiners Heimat Öster-reich, die vermuteten, dass der gerinnungshemmende Zusatz das Blut krankhaft verändern würde und eine Transfusion dieses Blutes gesundheitsschädlich sei. Sie zogen weiterhin die direkte Bluttransfusion vor oder suchten vergeblich weiter nach einer unkomplizierten Methode der Bluttransfusion, die sich flächendeckend anwenden ließ.
Während der Durchführung dieser Studie wurde offenbar, dass keiner der soeben ange-führten Aspekte als alleiniger Grund für die verspätete Rezeption der landsteinerschen Blutgruppen angesehen werden kann. Vielmehr war es ein Zusammenspiel der politischen Situation in Europa, der Rivalitäten unter Kollegen, die den Universitätsalltag beherrschten und nicht zuletzt die dargelegten Charakterzüge Landsteiners, die es ihm erschwerten, ein wissenschaftlich-soziales Netzwerk aufzubauen, das notwendig gewesen wäre, um seine Kollegen von der Bedeutung seiner Entdeckung zu überzeugen.
Different effects of conditional Knock-Out of Stat3 on the sensory epithelium of the Organ of Corti

The mammalian cochlea detects sound in response to vibration at frequency-dependent positions along the cochlea duct. The sensory outer hair cells, which are surrounded by supporting cells, act as a signal amplifier by changing their cell length. This is called electromotility. To ensure correct electrical transmission during mechanical forces, a certain resistance of the sensory epithelium is a prerequisite for correct transduction of auditory information. This resistance is managed by microtubules and its posttranslational modification in the supporting cells of the sensory epithelium of the cochlea. Stat3 is a transcription factor, with its different phosphorylation sites, is involved in many cellular processes like differentiation, inflammation, cell survival and microtubule dynamics, depending on cell type and activated pathway. While Stat3 has a wide range of intracellular roles, the question arose, how and if Stat3 is involved in cells of the organ of Corti to ensure a correct hearing.
To test this, Cre/loxp system were used to perform conditional Knock-Out (cKO) of Stat3 in outer hair cells or supporting cells either before hearing onset or after hearing onset. Hearing performances included DPOAE and ABR measurements, while molecular were performed by sequencing. Additionally, morphological examination was used by immunohistochemistry and electron microscopy.
A cKO of Stat3 before and after hearing onset in outer hair cells leads to hearing impairments, whereas synapses, nerve fibers and mitochondria were not affected. Bulk sequencing analyzation of outer hair cells out of cKO mice before hearing onset resulted in a disturbance of cellular homeostasis and extracellular signals. A cKO of Stat3 in the outer hair cells after hearing onset resulted in inflammatory signaling pathway with increased cytokine production and upregulation of NF-kb pathway. In supporting cells, cKO of Stat3 only after hearing onset resulted in a hearing impairment. However, synapses, nerve soma and fibers were not affected of a cKO of Stat3 in supporting cells. Nevertheless, detyronisated modification of microtubules were altered, which can lead to an instability of supporting cells during hearing.
In conclusion, Stat3 likely interact in a cell-specific and function-specific manner in cells of the organ of Corti. While a cKO in outer hair cells resulted in increased cytokine production, supporting cells altered its stability due to decreased detyronisated modification of microtubules. Together the results indicated that Stat3 is an important protein for hearing performances. However, additional investigations of the molecular mechanism are needed to understand the role of Stat3 in the cells of the organ of Corti.
mRNA is co- or post-transcriptionally processed from a precursor mRNA to a mature mRNA. In addition to 5'capping and splicing, these modifications also include polyadenylation, the addition of a polyA tail to the 3'end of the mRNA. In recent years, alternative polyadenylation in particular has increasingly been taken into account as a mechanism for regulating gene expression. It is assumed that approximately 70-75 % of human protein coding genes contain alternative polyadenylation signals, which are often located within intronic sequences of protein-coding genes. The use of such polyadenylation signals leads to shortened mRNA transcripts and thus to the generation of C-terminal shortened protein isoforms.
Interestingly, the majority of microRNAs, small non-coding RNAs that play an essential role in post-transcriptional gene regulation, are also encoded in intronic sequences of protein-coding genes and are co-transcriptionally expressed with their host genes. The biogenesis of microRNA has been well studied and is well known, but mechanisms that may influence the expression regulation of mature microRNAs are just poorly understood.
In the presented work, I aimed to investigate the influence of alternative intronic polyadenylation on the biogenesis of microRNAs. The human ion channel TRPM1 could already be associated with melanoma pathogenesis and truncated isoforms of this protein have already been described in literature. In addition, TRPM1 harbors a microRNA, miR211, in its sixth intron, which is assumed to act as a tumor suppressor. Since both, TRPM1 and miR211 have already been associated with melanoma pathogenesis, the shift towards truncated transcripts during the development of various cancers is already known and it has been shown that certain microRNAs play a crucial role in the development and progression of melanoma, melanoma cell lines were used as an in vitro model for these investigations.
Expanding on a general equilibrium model of offshoring, we analyze the effects of a unilateral emissions tax increase on the environment, income, and inequality. Heterogeneous firms allocate labor across production tasks and emissions abatement, while only the most productive can benefit from lower labor and/or emissions costs abroad and offshore. We find a non-monotonic effect on global emissions, which decline if the initial difference in emissions taxes is small. For a sufficiently large difference, global emissions rise, implying emissions leakage of more than 100%. The underlying driver is a global technique effect: While the emissions intensity of incumbent non-offshoring firms declines, the cleanest firms start offshoring. Moreover, offshoring firms become dirtier, induced by a reduction in the foreign effective emissions tax in general equilibrium. Implementing a BCA prevents emissions leakage, reduces income inequality in the reforming country, but raises inequality across countries.
Temporale Vokalisationseigenschaften von Neugeborenen mit intrauteriner Wachstumsretardierung

Die Zielstellung der vorliegenden Arbeit bestand in der Reflektion möglicher Einflüsse intrauteriner Wachstumsretardierungen auf die temporalen Vokalisationseigenschaften von Neugeborenen in den ersten fünf Lebenstagen. Unter der Annahme, dass das Gehirn des Neugeborenen das Schreiverhalten steuert, können die spezifischen Merkmale des Schreiens Aufschluss über die zugrunde liegende neuronale Integrität des Säuglings und eine eventuelle Prädisposition für Sprachanomalien geben. Zusätzlich wurden mögliche Einflüsse des Geburtsmodus, des Geschlechts und des Probanden- und Gestationsalters auf die temporalen Vokalisationseigenschaften untersucht. Dazu wurden spontane Lautäußerungen von 55 reifgeborenen, gesunden, neurologisch unbeeinträchtigten Neugeborenen mit einem Gestationsalter von 36-41 Wochen untersucht. Der Atemzyklus wurde gesamt und spezifisch für jedes Intervall betrachtet. 1605 Lautaufnahmen wurden vermessen und analysiert. Anhand standardisierter Wachstumskurven des Geburtsgewichtes erfolgte die somatische Klassifikation in SGA und AGA. Die Analysen ergaben signifikant verlängerte Zeitwerte der SGA-Neugeborenen für die Inspirationslänge, die Exspirationslänge und das Messintervall. Die hier berichteten Ergebnisse zeigen, dass signifikante Unterschiede bezüglich der Dauer einzelner zeitlicher Intervalle spontaner Vokalisationen zwischen SGA- und AGA-Neugeborenen bestehen. Das Verhältnis zwischen Inspirationslänge und Exspirationslänge hingegen erweist sich als robuste Größe, die sich nicht signifikant unterschied. Ein signifikanter Einfluss des Geburtsmodus auf die zeitliche Organisation von spontanen Lauten konnte in den Analysen nicht festgestellt werden. Jedoch waren die Zeitwerte der Messgrößen bei per Sectionem entbundenen Neugeborenen im Vergleich zu Spontangeburten tendenziell erhöht. Auch ein Einfluss des Geschlechts, des Probanden- oder Gestationsalters wurde für die temporalen Charakteristika nicht beobachtet.
Auf der Grundlage einer quantitativen Analyse untersucht Hans-Jürgen Brandt Konfliktlösungen und deren Wandel unter der Bedingung des Rechtspluralismus in andinen Gemeinschaften Perus und Ecuadors. Die indigene Justiz ist einerseits wegen ihrer friedensstiftenden und integrativen Funktion angesehen, sie ist andererseits aber nicht universalistisch orientiert und steht, beispielsweise im Hinblick auf Körperstrafen, zuweilen in Widerspruch zu den Menschenrechten.
Ein Hilfsangebot für die, die immer schon alle die interessanten Werke der neulateinischen Epik aus Italien lesen wollten, die heute so bequem im Internet stehen, aber nie die Zeit dafür fanden: Rund 50 wenig bekannte Epen aus fünf Jahrhunderten werden in griffiger Zusammenfassung des Inhalts geboten, mit grundlegenden Verständnishilfen und Hinweisen auf die besonders gelungenen wie auch auf weniger geglückte Stellen.
Die Darstellung wird hier in zweiter, korrigierter und ergänzter Auflage vorgelegt.

Abstract
Fabry disease (FD) is a life-limiting disorder characterized by intracellular globotriaosylceramide (Gb3) accumulations. The underlying α-galactosidase A (α-GAL A) deficiency is caused by variants in the gene GLA. Variants of unknown significance (VUS) are frequently found in GLA and challenge clinical management. Here, we investigated a 49-year old man with cryptogenic lacunar cerebral stroke and the chance finding of the VUS S126G, who was sent to our center for diagnosis and initiation of a costly and life-long FD-specific treatment. We combined clinical examination with in vitro investigations of dermal fibroblasts (HDF), induced pluripotent stem cells (iPSC), and iPSC-derived sensory neurons. We analyzed α-GAL A activity in iPSC, Gb3 accumulation in all three cell types, and action potential firing in sensory neurons. Neurological examination and small nerve fiber assessment was normal except for reduced distal skin innervation. S126G iPSC showed normal α-GAL A activity compared to controls and no Gb3 deposits were found in all three cell types. Baseline electrophysiological characteristics of S126G neurons showed no difference compared to healthy controls as investigated by patch-clamp recordings. We pioneer multi-level cellular characterization of the VUS S126G using three cell types derived from a patient and provide further evidence for the benign nature of S126G in GLA, which is of great importance in the management of such cases in clinical practice.
Ziel der vorliegenden Arbeit ist es, „Lanfranks ‚Chirurgia parva‘ in der Abschrift Konrad Schrecks von Aschaffenburg“1 anhand der von Ralf Vollmuth in seiner Habilitationsschrift „Traumatologie und Feldchirurgie an der Wende vom Mittelalter zur Neuzeit“ erarbeiteten Strukturvorgabe inhaltlich zu erschließen. Durch die Aufarbeitung verschiedener chirurgischer Fachbücher und Manuale unter Verwendung einer gemeinsamen Strukturvorlage soll ermöglicht werden, medizinhistorische Quellen kritisch-kontrastiv zu vergleichen. Das bedeutet, dass die Quellen zuerst ediert und anschließend gegebenenfalls übersetzt werden müssen. Im nächsten Schritt werden die verwendeten Arzneimittel – pflanzlicher, tierischer, mineralischer Herkunft – identifiziert und bestimmt. Im Anschluss werden Monographien mit den bestimmenden Inhaltsstoffen und Eigenschaften
erstellt. Anhand dieser Pflanzen- und Arzneistoffmonographien, die im Sinne einer Datenbank aufeinander aufbauen, sollte es dann möglich sein, unter modernen pharmakologischen Gesichtspunkten die Wirksamkeit der verwendeten Arzneimittel zu erschließen.
Eine ausreichende Zahl von Quellen, die nach einer gemeinsamen Strukturvorlage bearbeitet wurden, kann es schließlich ermöglichen, zu beurteilen, welche der beschriebenen Anwendungen repräsentativ waren, welche Außenseiterstellung einnahmen oder nur theoretische Ansätze bildeten, die praktisch keine Verwendung fanden.
Auf der Suche nach Frieden. Zur Gestaltung von Nachkriegsordnungen – eine archäologische Spurensuche

Ein in der archäologischen Konfliktforschung bislang wenig beachtetes Thema, die Geschehnisse nach Kampfhandlungen und die Etablierung einer Nachkriegsordnung, verbunden mit der Entstehung symbolischer Landschaften als Element einer Erinnerungskultur, behandelt Stefan Burmeister, der unter anderem die Ereignisse nach dem sog. Karfreitagsgefecht der Bundeswehr am 2. April 2010 in Afghanistan und die Rolle der materiellen Kultur in diesem Kontext analysiert.
Das frühzeitige Erkennen psychoonkologischer Belastungen ist Bestandteil des optimalen therapeutischen Managements von Tumorpatienten. Nur wenige, widersprüchliche Studien untersuchten bisher das psychische Befinden im Verlauf einer PET/CT. Bezogen auf das Prostatakarzinom gibt es bislang keine spezifische Studie, obwohl es die häufigste onkologische Erkrankung des Mannes darstellt. Aufgrund der insgesamt guten Prognose wird von einer geringeren psychischen Belastung ausgegangen. Mithilfe dieser Studie sollte durch Kombination etablierter Fragebögen das psychische Befinden im Verlauf der PET/CT explorativ untersucht werden.
Von Oktober 2018 bis Februar 2020 wurde 531 männlichen Patienten der Nuklearmedizin des Universitätsklinikums Würzburg die Teilnahme angeboten. N = 85 Patienten (n = 38 Patienten mit Prostatakarzinom sowie n = 47 Patienten mit anderen malignen Erkrankungen) stimmten einer vollständigen Teilnahme zu. Es wurden zwei Messzeitpunkte (T1 nach Durchführung der PET/CT; T2 nach Ergebnismitteilung) festgelegt. Als Messinstrumente wurden der PA-F-KF, QUICC, DT, STAI-X1, PANAS und ein Selbsteinschätzungsbogen verwendet.
24 % (T1) bzw. 35 % (T2) der Patienten mit Prostatakarzinom gaben eine dysfunktionale Progredienzangst an, 55 % (T1+T2) eine pathologische psychische Belastung. 53 % (T1) bzw. 50 % (T2) der Patienten zeigten eine relevant erhöhte Zustandsangst.
Die Progredienzangst stieg nach Ergebnismitteilung an (p = 0,048; η² = 0,106), die Ungewissheit über den Stand der Erkrankung (p = 0,014; η² = 0,165) und Bewältigbarkeit des Alltags (p = 0,016; η² = 0,163) reduzierten sich. Allgemeine Ängste wie die Zustandsangst, der Distress und negative Affekte veränderten sich nicht. PSA-Werte ohne bildmorphologisches Korrelat lösten eine größere Unsicherheit bezüglich des aktuellen Krankheitszustandes aus (p = 0,029; η² = 0,128). Jüngere Patienten zeigten vor (p = 0,005; η² = 0,207) und nach (p = 0,001; η² = 0,290) Ergebnismitteilung eine höhere Angst um ihre Berufstätigkeit und gaben eine geringere Erleichterung nach Ergebnismitteilung (p = 0,016; η² = 0,165) an.
Als Limitationen sind die geringe Fallzahl und Teilnahmequote, multiple Testung und fehlende Erfragung psychischer Erkrankungen zu beachten.
Insgesamt zeigen sich eine hohe psychische Belastung und Ängste im Verlauf der PET/CT. Patienten mit Prostatakarzinom sind zu diesem Zeitpunkt nicht weniger belastet als Patienten mit anderen malignen Erkrankungen.
Postoperative Übelkeit und postoperatives Erbrechen (PONV) sind eine der häufigsten und für Patient*innen unangenehmsten Nebenwirkungen einer Allgemeinanästhesie. Trotz jahrzehntelanger Forschung und der Vielfalt an mittlerweile bekannten Maßnahmen und Substanzen zur PONV-Prophylaxe und -Therapie gibt es noch keine Strategie, die eine sichere Vermeidung oder stets wirksame Therapie von PONV garantieren kann. In vorangegangenen Studien zeigte Amisulprid als Dopaminantagonist an den Rezeptortypen D2 und D3 vielversprechende Ergebnisse zur PONV-Prophylaxe und -Therapie.
Die dieser Arbeit zugrunde liegende prospektive, randomisierte, Placebo-kontrollierte Doppelblindstudie untersuchte die Wirksamkeit einer Einzeldosis APD421 5 mg bzw. 10 mg zur Therapie von manifestem PONV nach fehlgeschlagener PONV-Prophylaxe. „Complete Response“ (CR) wurde definiert als das Ausbleiben jeglicher weiterer emetischer Episoden im Zeitraum von 30 Minuten bis 24 Stunden nach Applikation des Studienmedikaments sowie keine Gabe von antiemetischer Rescue-Medikation im gesamten Zeitraum bis 24 Stunden nach Applikation des Studienmedikaments. Die CR-Raten lagen bei 41,7% für APD421 10 mg, 33,8% für APD421 5 mg und 28,5% für Placebo, wobei die Studienarme jeweils 230, 237 bzw. 235 Patient*innen umfassten. Eine Dosis APD421 10 mg zeigte somit statistisch signifikante Überlegenheit in der PONV-Therapie gegenüber Placebo. Auch hinsichtlich sekundärer Studienendpunkte wie Auftreten bzw. Stärke von Übelkeit, Würgen oder Erbrechen und Bedarf an Rescue-Medikation war APD421 10 mg gegenüber Placebo überlegen. Eine Dosis von 5 mg APD421 zeigte für die meisten Endpunkte hingegen keine statistisch signifikante Überlegenheit gegenüber Placebo.
Limitationen der Studie liegen im Ausschluss von Patientengruppen wie beispielsweise Kindern oder bestimmten Vorerkrankungen und dem mit über 90% sehr hohen Anteil weiblicher Patient*innen. Es bleiben weitere Studien abzuwarten, die APD421 einem direkten Vergleich mit bislang etablierten Substanzen zur PONV-Therapie unterziehen, um den künftigen Stellenwert der Substanz im klinischen Alltag einschätzen zu können.
Einleitung:
In dieser Arbeit wurde die Auswirkung der Fettgewebesurrogate Halsumfang (HU), Taillenumfang (TU) und Body Mass Index (BMI) auf die Prognose bei Patienten mit chronischer Niereninsuffizienz untersucht.
Methoden:
Datengrundlage dieser Arbeit war die German Chronic Kidney Disease (GCKD) Beobachtungsstudie. Eingeschlossen wurden Erwachsene mit GFR 30-60 ml/min/1,73m² oder GFR > 60 ml/min/1,73m² mit offensichtlicher Proteinurie. Ausschlusskriterien waren: nicht-kaukasische Ethnie, Organtransplantation, Malignome und Herzinsuffizienz NYHA IV. Untersuchte kombinierte Endpunkte (EP) waren: 1) 4P-MACE (Herzinfarkt, Schlaganfall, kardiovaskulärer Tod, pAVK-Ereignis) 2) Tod jeglicher Ursache 3) Nierenversagen (Dialyse, Transplantation). Es wurden Cox-Regressionen mit HU, TU, und BMI für jeden EP, adjustiert für Alter, Geschlecht, Nikotinkonsum, Diabetes mellitus, arterielle Hypertonie, LDL-Cholesterin, GFR, Urin-Albumin/Kreatinin Ratio (UACR) und CRP berechnet. Interaktionsterme des jeweiligen Surrogats mit dem Geschlecht wurden eingeschlossen.
Ergebnisse:
Von den 4537 analysierten Studienteilnehmern, waren 59% Männer mit einem Durchschnittsalter von 60 (±12) Jahren, einer mittleren GFR von 50 (±18) ml/min/1,73m² und einem UACR-Median von 49 (10–374) mg/g. Der mittlere HU war 42,7 (±3,6) cm bei Männern und 37,2 (±3,7) cm bei Frauen, der mittlere TU 107,6 (±13,6) cm bei Männern und 97,0 (±16,3) cm bei Frauen und der mittlere BMI 29,7 (±5,9) kg/m². Die mittlere Beobachtungszeit betrug 6,5 Jahre. Der TU war signifikant mit Tod assoziiert, mit einer HR von 1,014 pro cm (95% KI 1,005–1,024). HU war signifikant mit Tod bei Frauen assoziiert, Interaktionsterm HR 1,080 pro cm (95% KI 1,009–1,155). Der BMI hatte keinen signifikanten Einfluss auf untersuchte EP.
Schlussfolgerung:
Bei Patienten mit mittel- bis schwergradig eingeschränkter Nierenfunktion steigern ein erhöhter TU (bei beiden Geschlechtern), sowie bei Frauen ein erhöhter HU das Risiko für Tod jeglicher Ursache.
Die Na+ /K+ -ATPase (NKA) ist maßgeblich an der Regulation der kardialen Na+ -Homöostase beteilligt. Im Myokard werden hauptsächlich zwei Isoformen exprimiert: die α1 (NKA-α1) und die α2-Isoform (NKA-α2). Diese beiden Isoformen unterscheiden sich sowohl in ihrer Lokalisation als auch in ihrer zellulären Funktion. So ist die NKA-α1 recht homogen entlang des Sarkolemms zu finden und ist verantwortlich für die Regulation der globalen intrazellulären Na+ -Konzentration ([Na+ ]i). Die NKA-α2 hingegen konzentriert sich hauptsächlich in den T-Tubuli und beeinflusst über Veränderung der lokalen [Na+ ]i die Ca2+ -Transienten und die Kontraktilität. Im Rahmen einer Herzinsuffizienz wurde eine verminderte Expression und Aktivität der NKA beobachtet. Gleichzeitig werden Inhibitoren der NKA, sogenannte Digitalisglykoside, in fortgeschrittenen Herzinsuffizienz-Stadien eingesetzt. Die Studienlage über den Einsatz dieser Therapeutika ist recht uneinheitlich und reicht von einer verringerten Hospitalisierung bis hin zu einer erhöhten Mortalität. Ziel dieser Arbeit war es die Folgen einer NKA-α2 Aktivierung während einer Herzinsuffizienz mit Hilfe eines murinen Überexpressionsmodells zu analysieren. 11-Wochen alte Mäuse mit einer kardialen NKA-α2 Überexpression (NKA-α2) und Wildtyp (WT) Versuchstiere wurden einem 8-wöchigen Myokardinfarkt (MI) unterzogen. NKA-α2 Versuchstiere waren vor einem pathologischem Remodeling und einer kardialen Dysfunktion geschützt. NKA-α2 Kardiomyozyten zeigten eine erhöhte Na+ /Ca2+ -Austauscher (NCX) Aktivität, die zu niedrigeren diastolischen und systolischen Ca2+ -Spiegeln führte und einer Ca2+ -Desensitisierung der Myofibrillen entgegenwirkte. WT Versuchstiere zeigten nach chronischem MI eine sarkoplasmatische Ca2+ -Akkumulation, die in NKA-α2 Kardiomyozyten ausblieb. Gleichzeitig konnte in der NKA-α2 MI Kohorte im Vergleich zu den WT MI Versuchstieren eine erhöhte Expression von β1-adrenergen Rezeptoren (β1AR) beobachtet werden, die eine verbesserte Ansprechbarkeit gegenüber β-adrenergen Stimuli bewirkte. Zudem konnte in unbehandelten Versuchstieren eine Interaktion zwischen NKA-α2 und dem β1AR nachgewiesen werden, welche in der WT Kohorte größer ausfiel als in der NKA-α2 Versuchsgruppe. Gleichzeitig zeigten unbehandelte NKA-α2 Kardiomyozyten eine erhöhte Sensitivität gegenüber β-adrenerger Stimulation auf, welche nicht mit einer erhöhten Arrhythmie-Neigung oder vermehrten Bildung reaktiver Sauerstoffspezies einherging. Diese Untersuchungen zeigen, dass eine NKA-α2 Überexpression vor pathologischem Remodeling und einer kardialen Funktionbeeinträchtigung schützt, indem eine systolische, diastolische und sarkoplasmatische Ca2+ -Akkumulation verhindert wird. Gleichzeitig wird die β1AR Expression stabilisert, wodurch es zu einer verminderten neurohumoralen Aktivierung und einer Durchbrechung des Circulus vitiosus kommen könnte. Insgesamt scheint eine Aktivierung der NKA-α2 durchaus ein vielversprechendes Target in der Herzinsuffizienz Therapie darzustellen.

Virtual humans (VHs) hold immense potential for collaboration in social virtual reality (VR). As VR technology advances, it's vital to assess the psychological effects on VH trust and user privacy to build meaningful social interactions in VR. In social VR, users must be able to trust the VHs they interact with as they navigate through socio-cultural activities. The evaluation of trustworthiness in VHs profoundly impacts interaction quality and user willingness to engage. Conversely, untrustworthy VHs can harm user experiences, privacy, and VR engagement. To address this, we conducted immersive VR studies, exploring how psychological factors influence user's VH trust evaluation under various psychological conditions. This research is pivotal for developing strategies to enhance user privacy, establish secure VR environments, and create a foundation of trust that supports immersive socio-cultural experiences in VR.
To date, there are no established interpersonal trust measurement tools specifically for VHs in VR. In study 1 (the familiarity study) of the current thesis the VR-adjusted version of the social conditioned place preference paradigm (SCPP) by Kiser et al., (2022) was identified as a potential trust measurement tool. We tested whether the familiarity of a VH influenced trust as measured with the SCPP paradigm and other self-defined outcome measures, in a Computer Augmented Virtual Environment (CAVE). The CAVE is a VR system that combines immersive VR with real-world elements. It consists of a room-sized space where the walls are used as projection screens to display virtual scenes and objects. In this within - subject design (n = 20), half of the participants were familiarized with one VH and tasked to explore and interact in a realistic looking virtual art museum environment. The participant’s evaluation of the VH’s trustworthiness was measured as well as their subsequent trust behaviours. Results revealed no significant differences in the evaluation of the VH’s trustworthiness nor any behavioural differences between conditions. The findings of the impact of a VH’s familiarity on trust is inconclusive due to the major limitations of the paradigm. We concluded that the SCPP paradigm needs further validation and the proposed proxies of trust need to be re-evaluated. The findings were considered in the following study.
The virtual maze paradigm design of Hale, (2018) was identified as a potential trust measurement tool, however several limitations are associated with its use to measure trust in VR. In study 2 (a validation study), improvements were made to the virtual maze paradigm of Hale, (2018) and a variant of this paradigm was implemented. We conducted a validation study with 70 participants in a between-subject design with VH trustworthiness as the between-subject factor. Participants wore a head-mounted display (HMD), to deliver an immersive VR experience. In our version of the virtual maze, it was the task of the users (the trustors) to navigate through a maze in VR, where they could interact with a VH (the trustee). They could choose to ask for advice and follow the advice from the VH if they wanted to. The number of times participants asked and followed advice and the time it took to respond to the given advice served as behavioural proxies/measures of trust. The two conditions (trustworthy vs. untrustworthy) did not differ in the content of the advice but in the appearance, tone of voice and engagement of the trustees (allegedly an avatar controlled by other participants). Results indicated that the experimental manipulation was successful, as participants rated the VH as more trustworthy in the trustworthy condition compared with the VH in the untrustworthy condition. Importantly, this manipulation affected the trust behaviour of participants, who, in the trustworthy condition, asked for advice and followed advice more often, indicating that the paradigm is sensitive to differences in VH’s trustworthiness. Thus, our paradigm can be used to measure differences in interpersonal trust towards VHs and may serve as a valuable research tool for researchers who study trust in VR. Therefore, study 2 fills the gap in the literature, for an interpersonal trust measurement tool specifically for VHs in VR.
Two experimental studies, with a sample size of 50 participants each, utilized the virtual maze paradigm where participants entered 12 rooms under different conditions. We examined the influence of cognitive load (CL) on trust towards VH in VR in study 3 (Cognitive load study), and the influence of emotional affect (Emotional affect study) on trust towards VH in VR in study 4 (EA study). In both studies, we assessed participant’s evaluation of a VH’s trustworthiness, along with three behavioural indicators of trust in the maze task: 1) frequency of advice asked, 2) frequency of advice followed, and 3) the time taken by participants to execute the received advice. In study 3, the CL was manipulated with the auditory 1-back task in the high cognitive load condition (HCL). In study 4, the Autobiographical Emotional Memory Task (AEMT) was used to manipulate the EA of participants in the negative emotional affect (NEA) condition. As an additional manipulation, while participants were immersed in VR, they were exposed to 12 negative pictures and sounds that was presented simultaneously to strengthen the initial manipulation. The manipulation of the within-subject factors (CL and EA) was successful in both studies, as significant differences between conditions were observed in both studies (higher CL in the HCL condition and a more negative EA in the NEA condition). However, only CL influenced participant’s evaluation of the VH’s trustworthiness. The VH were evaluated as significantly more trustworthy after the HCL condition. Despite the difference in trust evaluation, there was no difference in advice asking or following. Participants in study 4 asked and followed advice due to their trust in the VH and asked and followed advice equally often in both conditions. Importantly, significant differences were observed in the participants response times in both studies. In study 3 during the HCL condition participants followed advice quicker. The order in which the conditions were presented influenced the experience of CL. Participants experienced higher levels of CL and responded to advice significantly faster when low cognitive load (LCL) was presented as the first condition compared with LCL as the second condition. In study 4 participants in the NEA condition followed advice slower similar to the findings of study 3. The order in which the conditions were presented had a significant effect on the EA. Participants asked and followed advice less when the NEA condition was presented first compared with when it is presented second. Possible explanations for the findings are discussed in the thesis.
Overall, this thesis offers a novel tool for trust measurement (the virtual maze paradigm) and contributes to understanding the role of psychological factors in trust towards virtual humans in virtual reality.
Proteins fold in water and achieve a clear structure despite a huge parameter space. Inside a (protein) crystal you have everywhere the same symmetries as there is everywhere the same unit cell. We apply this to qubit interactions to do fundamental physics:
We modify cosmological inflation: we replace the big bang by a condensation event in an eternal all-encompassing ocean of free qubits. Rare interactions of qubits in the ocean provide a nucleus or seed for a new universe (domain), as the qubits become decoherent and freeze-out into defined bit ensembles. Next, we replace inflation by a crystallization event triggered by the nucleus of interacting qubits to which rapidly more and more qubits attach (like in everyday crystal growth). The crystal unit cell guarantees same symmetries (and laws of nature) everywhere inside the crystal, no inflation scenario is needed.
Interacting qubits solidify, quantum entropy decreases in the crystal, but increases outside in the ocean. The interacting qubits form a rapidly growing domain where the n**m states become separated ensemble states, rising long-range forces stop ultimately further growth. After this very early modified steps, standard cosmology with the hot fireball model takes over. Our theory agrees well with lack of inflation traces in cosmic background measurements.
Applying the Hurwitz theorem to qubits we prove that initiation of qubit interactions can only be 1,2,4 or 8-dimensional (agrees with E8 symmetry of our universe). Repulsive forces at ultrashort distances result from quantization, long-range forces limit crystal growth. The phase space of the crystal agrees with the standard model of the basic four forces for n quanta. It includes all possible ensemble combinations of their quantum states m, a total of n**m states. We describe a six-bit-ensemble toy model of qubit interaction and the repulsive forces of qubits for ultra-short distances. Neighbor states reach according to transition possibilities (S-matrix) with emergent time from entropic ensemble gradients. However, in our four dimensions there is only one bit overlap to neighbor states left (almost solid, only below Planck´s quantum is liquidity left). The E8 symmetry of heterotic string theory has six curled-up, small dimensions. These keep the qubit crystal together and never expand. We give energy estimates for free qubits vs bound qubits, misplacements in the qubit crystal and entropy increase during qubit crystal formation.
Implications are fundamental answers, e.g. why there is fine-tuning for life-friendliness, why there is string theory with rolled-up dimension and so many free parameters. We explain by cosmological crystallization instead of inflation the early creation of large-scale structure of voids and filaments, supercluster formation, galaxy formation, and the dominance of matter: the unit cell of our crystal universe has a matter handedness avoiding anti-matter. Importantly, crystals come and go in the qubit ocean. This selects for the ability to lay seeds for new crystals, for self-organization and life-friendliness. Vacuum energy gets appropriate low inside the crystal by its qubit binding energy, outside it is 10**20 higher. Scalar fields for color interaction/confinement and gravity could be derived from the qubit-interaction field.
The demand for LIB with enhanced energy densities leads to increased utilization of the space within the confinements of the battery housing or to the use of electrode material with increased intrinsic specific energy densities. Both requirements result in more stress on the battery electrodes and separator during cycling or aging. However, the effect of mechanical strain on the cell’s electrochemistry and thus the performance of batteries is rather unexplored compared to the impact of current or temperature, for example. The objective of this thesis was to give a better understanding of the electrochemical and mechanical interplay in current- and next-generation lithium based battery cells. Therefore, the thesis was structured into the investigations on SoA and next-generation LIBs. For SoA LIBs, the investigations of the interplay started at laboratory scale. Here, the expansion of various electrodes and also the impact of mechanical pressure and its distribution on the performance of the cells were
studied. The investigations at laboratory scale was followed by an examination of the electrochemical and mechanical interactions on large format commercial LIBs which are used in BEVs. Accordingly, the effect of bracing and its effect on the performance was studied in an aging and post-mortem study. To gain a deeper understanding of the mechanical changes in LIBs, an ultrasonic study was performed for pouch cells. Here, the mechanical changes were further investigated in dependence of SoC and SoH. The effects of the mechanical stress on the performance for next-generation batteries were studied at laboratory scale. In the beginning, the expansion of next-generation anode materials such as silicon and lithium was compared with today’s anode materials. Furthermore, the effect of mechanical pressure and electrolyte on the irreversible dilation and performance was investigated for lithium metal cells. Overall, it was shown that pressure has a significant effect on the performance of today’s and also future LIBs. The interplay of the electrochemical and mechanical effects inside a LIB has a considerable impact on the lifetime, capacity fading and impedance increase of the batteries.
71 Studierende nahmen am Universitätsklinikum Würzburg in der Abteilung für Zahnärztliche Prothetik an einem freiwilligen Übungsseminar zum Aufpassen von Kronen mit Störstellen, die im 3D-Druckverfahren hergestellt wurden, teil. Das Übungsseminar fand an zwei Terminen statt. Zum Identifizieren der Störstellen standen Xantopren und Okklusionsspray zur Verfügung. Nach dem praktischen Teil der Übung wurde ein Fragebogen ausgefüllt. Zusätzlich wurden die aufgepassten Kronen mittels Laborscanner digitalisiert und mit einer Krone ohne Störstellen überlagert. Dadurch konnten positive und negative Oberflächenabweichungen für die Bereiche der Störstellen sowie der Gesamtinnenfläche der Kronen ermittelt werden.
Die flächenbezogenen Abweichungswerte zeigten einen signifikanten Lernerfolg – gemessen anhand der Passungsparameter - zwischen den beiden Terminen des Übungsseminars. Hierbei erreichten Kronen, die mit Okklusionsspray aufgepasst wurden, signifikant geringere flächenbezogene Abweichungswerte im Vergleich zu Kronen, die mit Xantopren aufgepasst wurden.
Die Auswertung der mit Schulnoten skalierten Fragen ergab signifikante Unterschiede bei der Bewertung der Härte, eines realitätsnahen Gefühls beim Einschleifen bzw. beim Aufpassen und Details wie Randschluss. Beim Vergleich der Aufpassmethoden im Fragebogen ergaben die Einfachheit beim Aufpassen, das Identifizieren der Störstellen und das präferierte Material signifikante Unterschiede. Der subjektive Lernerfolg mit den Materialien zeigte ebenfalls signifikante Unterschiede. Insbesondere die Materialeigenschaften und die Randgenauigkeit der Druckkronen wurden häufig kritisiert, die schnelle und einfache Möglichkeit zur Herstellung von Übungsmaterialien sowie deren Reproduzierbarkeit wurden von den Studierenden hingegen begrüßt.
ICIs sind inzwischen integrales Therapiemittel vieler Tumoren, selbst in nicht metastasierten Stadien. Das Management von dabei eventuell entstehenden Nebenwirkungen bleibt wichtiger Bestandteil der Therapie vor allem im fortgeschrittenen Alter. Retrospektive Untersuchungen wie unsere tragen dazu bei, das in vielen klinischen Studien unterrepräsentierte Patientenkollektiv älterer Patienten in den klinischen Alltag sowie in Therapieentscheidungen und -planungen zu integrieren.
Der primäre Studienendpunkt unserer Arbeit unterstützt wichtige Erkenntnisse anderer Studien, dass irAEs insgesamt unter älteren Patienten nicht häufiger auftreten. Zwischen allen drei Altersklassen von ~55, ~70 und ~80 Jahren zeigten sich keine signifikanten Unterschiede im Auftreten von irAEs aller Grade, wobei irAEs Grad III/IV etwas häufiger bei ~80-Jährigen auftraten. In unserem Fall stellten wir fest, dass auftretende irAEs im Alter häufiger behandelt wurden, und dass die Immuntherapie häufiger pausiert oder abgebrochen wurde. Zudem war der Anteil an Therapieabbrüchen unter den älteren Patienten wegen bestimmter Ereignisse wie TRAEs und dem Einsatz von Glukokortikoiden höher als bei jüngeren Patienten.
Die Ergebnisse unserer Studie deuten außerdem darauf hin, dass selbst unter Polypharmazie und Multimorbidität irAEs nicht häufiger bei Älteren auftraten. Ebenso können wir die interessante Beobachtung verzeichnen, dass Patienten mit >5 Medikamenten und gleichzeitig >5 Erkrankungen signifikant mehr irAEs Grad III/IV aufwiesen oder mehr Patienten Glukokortikoide verabreicht bekommen haben. Auch der Anteil an Interventionsbedarf oder Therapieabbruch war hier in allen Altersklassen höher. Es stellt sich die Frage, inwiefern hohes Alter, Komorbidität und Polypharmazie Risikofaktoren für Interventionsbedarf oder Therapieabbruch in der Immuntherapie sind, und ob ihnen eher besondere Gewichtung als Risikofaktor zukommt als dem Alter selbst.
Obwohl es in den letzten 10-15 Jahren gelang, multiple MM-Genome mittels NGS auf eine kosteneffiziente Art und mit geringem Zeit- und Materialaufwand zu sequenzieren und hierdurch zum Teil bahnbrechende Erkenntnisse gewonnen werden konnten, sind molekulargenetische Untersuchungen im diagnostischen Workflow des MMs bisher nicht ausreichend implementiert, um eine personalisierte Therapieentscheidung zu ermöglichen.
Vor diesem Hintergrund wurde in der vorliegenden Arbeit eine Gruppe an Patienten mit NDMM und RRMM anhand klinischer Parameter charakterisiert und durch Verwendung des M³P-Panels auf das Vorliegen bestimmter molekulargenetischer Veränderungen untersucht. Zusammenfassend lässt sich sagen, dass unsere Analyse die bisher veröffentliche M³P-Prävalenz in MM-Tumorproben bestätigt. Zu den am häufigsten mutierten Genen gehörten KRAS, NRAS, DIS3, ATM und BRAF. In der Gruppe der Patienten mit NRAS-Mutation oder del17p war die Zahl der relevanten Mutationen deutlich höher als ohne Vorliegen der entsprechenden Veränderung. Der Nachweis eines Double-Hit-Myeloms war erwartungsgemäß der stärkste ungünstige Faktor in unserer Kohorte. Unter den Patienten mit CRBN-Mutation waren alle IMiD-vorbehandelt und zeigten im Verlauf eine Refraktärität gegenüber dieser Substanzgruppe auf. Bezüglich der Überlebensanalysen bestätigten unsere Ergebnisse bereits bekannte prognostische Risikofaktoren wie Hochrisikozytogenetik, insbesondere del17p und gain1q, eine TP53-Mutation sowie ISS- und R-ISS-Stadium III.
Die Ergebnisse der Mutationsanalysen dieser Arbeit verdeutlichen den großen wissenschaftlichen und therapeutischen Nutzen, der von molekulargenetischen Untersuchungen ausgeht. Zukünftig werden auch beim MM Therapieentscheidungen auf Grundlage genetischer Diagnostik getroffen werden, mit dem Ziel die Behandlung für MM-Patienten weiter zu verbessern.
Magnetic Particle Imaging (MPI) ist ein innovatives tomographisches Bildgebungsverfahren, mit dem Tracerpartikel äußerst sensitiv und schnell mehrdimensional abgebildet werden können. Die Methode basiert auf der nichtlinearen Magnetisierungsantwort superparamagnetischer Eisenoxidnanopartikel (SPION) in einem Messpunkt, welcher ein Messvolumen rastert. In vorliegender Arbeit wurde das sog. Traveling Wave MPI (TWMPI) Verfahren eingesetzt, wodurch im Vergleich zu konventionellen MPI-Scannern ein größeres Field of View (FOV) und eine geringere Latenz bis zur Bildanzeige erreicht werden konnte. TWMPI weist einige für medizinische Zwecke vielversprechende Eigenschaften auf: Es liefert zwei- und dreidimensionale Bildrekonstruktionen in Echtzeit mit hoher zeitlicher und räumlicher Auflösung. Dabei ist die Bildgebung von Grund auf hintergrundfrei und erfordert keinerlei ionisierende Strahlung. Zudem ist die Technik äußerst sensitiv und kann SPION-Tracer noch in mikromolaren Konzentrationen detektieren.
Ziel dieser Arbeit war es daher zu untersuchen, inwiefern es mittels TWMPI möglich ist, künstliche Stenosen im Gefäßmodell visuell in Echtzeit darzustellen und quantitativ zu beurteilen sowie überdies eine perkutane transluminale Angioplastie (PTA) im Gefäßmodell unter TWMPI-Echtzeit-Bildgebung durchzuführen.
Alle Experimente wurden in einem speziell angefertigten TWMPI-Scanner durchgeführt (JMU Würzburg, Experimentelle Physik V (Biophysik), FOV: 65 x 29 x 29 mm³, Auflösung: ca. 1.5 - 2 mm). Die Lumen-Darstellungen erfolgten mittels des SPION-Tracers Ferucarbotran in einer Verdünnung von 1 : 50 (entspr. 10 mmol [Fe]/l). Das PTA-Instrumentarium wurde mit eigens hergestelltem ferucarbotranhaltigem Lack (100 mmol [Fe]/l) markiert. Für die verschiedenen Teilexperimente wurden den jeweiligen speziellen Anforderungen entsprechend mehrere Gefäßmodelle handgefertigt.
Für die visuelle Stenosequantifizierung wurden fünf starre Stenosephantome unterschiedlicher Stenosierung (0%, 25%, 50%, 75%, 100%) aus Polyoxymethylen hergestellt (l: 40 mm, ID: 8 mm). Die Gefäßmodelle wurden mehrfach zentral im FOV platziert und das stenosierte Lumen mittels sog. Slice-Scanning Modus (SSM, Einzelaufnahme inkl. 10 Mittelungen: 200 ms, Bildfrequenz: 5 Bilder pro Sekunde, Latenz: ca. 100 ms) als zweidimensionale Quasi-Projektionen abgebildet. Diese Aufnahmen (n = 80, 16 je Phantom) wurden mit einer einheitlichen Grauskalierung versehen und anschließend entsprechend den NASCET-Kriterien visuell ausgewertet.
Alle achtzig Aufnahmen waren unabhängig vom Stenosegrad aufgrund einheitlicher Fensterung sowie konstanter Scannerparameter untereinander gut vergleichbar. Niedriggradige Stenosen konnten insgesamt genauer abgebildet werden als höhergradige, was sich neben der subjektiven Bildqualität auch in geringeren Standardabweichungen zeigte (0%: 3.70 % ± 2.71, 25%: 18.64 % ± 1.84, 50%: 52.82 % ± 3.66, 75%: 77.84 % ± 14.77, 100%: 100 % ± 0). Mit zunehmendem Stenosegrad kam es vermehrt zu geometrischen Verzerrungen im Zentrum, sodass bei den 75%-Stenosen eine breitere Streuung der Messwerte mit einer höheren Standardabweichung von 14.77% einherging. Leichte, randständige Artefakte konnten bei allen Datensätzen beobachtet werden.
Für die PTA wurden drei interaktive Gefäßmodelle aus Polyvinylchlorid (l: 100 mm, ID: 8 mm) mit zu- und abführendem Schlauchsystem entwickelt, welche mittels Kabelband von außen hochgradig eingeengt werden konnten. Analog zu einer konventionellen PTA mittels röntgenbasierter digitaler Subtraktionsangiographie (DSA), wurden alle erforderlichen Arbeitsschritte (Gefäßdarstellung, Drahtpassage, Ballonplatzierung, Angioplastie, Erfolgskontrolle) unter (TW)MPI-Echtzeit-Bildgebung (Framerate: 2 - 4 FPS, Latenz: ca. 100 ms) abgebildet bzw. durchgeführt.
Im Rahmen der PTA war eine Echtzeit-Visualisierung der Stenose im Gefäßmodell durch Tracer-Bolusgabe sowie die Führung des markierten Instrumentariums zum Zielort möglich. Die Markierung der Instrumente hielt der Beanspruchung während der Prozedur stand und ermöglichte eine genaue Platzierung des Ballonkatheters. Die Stenose konnte mittels Angioplastie-Ballons unter Echtzeit-Darstellung gesprengt werden und der Interventionserfolg im Anschluss durch erneute Visualisierung des Lumens validiert werden.
Insgesamt zeigt sich MPI somit als adäquate Bildgebungstechnik für die beiden in der Fragestellung bzw. Zielsetzung definierten experimentellen Anwendungen. Stenosen im Gefäßmodell konnten erfolgreich in Echtzeit visualisiert und bildmorphologisch nach NASCET-Kriterien quantifiziert werden. Ebenso war eine PTA im Gefäßmodell unter TWMPI-Echtzeit-Bildgebung machbar. Diese Ergebnisse unterstreichen das grundlegende Potenzial von MPI für medizinische Zwecke. Um zu den bereits etablierten Bildgebungsmethoden aufzuschließen, ist jedoch weitere Forschung im Bereich der Scanner-Hard- und -Software sowie bezüglich SPION-Tracern nötig.
Martin Dinges’ Konzept der „Justiznutzung“ meint eine Instrumentalisierung der Justiz beispielsweise mittels einer gleichzeitigen oder alternativen Nutzung verschiedener justizförmiger Angebote nach Opportunitätskriterien. Ausgehend von diesem Konzept, ermittelt er Gemeinsamkeiten mit und Differenzen zu der Nutzung medizinischer Angebote in der europäischen Frühmoderne.
Laut des aktuellen Reports der Weltgesundheitsorganisation sind ca. 466 Millionen Menschen weltweit von einer Hörstörung (HS) betroffen. Durch die enorme Heterogenität und die klinische Variabilität, die diese Erkrankung ausmacht, und viele bisher nicht mit HS assoziierte Gene, bleibt ein großer Teil der erblich bedingten HS in vielen Familien unaufgeklärt. Die Entwicklung moderner Techniken, wie die Next-Generation Sequenzierung (NGS) und der Fortschritt bei der Untersuchung von Modellorganismen trugen jedoch in den letzten Jahren immens dazu bei, neue Gene zu identifizieren, die innerhalb des auditorischen Signalwegs oder damit assoziierten Strukturen beteiligt sind. Die vorliegende Arbeit umfasst Ergebnisse dreier Veröffentlichungen, in denen iranische und pakistanische Familien und eine deutsche Familie mit erblich bedingter HS untersucht und neue, krankheitsverursachende Varianten identifiziert und funktionell charakterisiert wurden. Im ersten Abschnitt konnten zwei neue rezessive Varianten im CDC14A-Gen als krankheitsverursachend identifiziert werden, die zu einem potentiellen Funktionsverlust des kodierten Proteins in einer iranischen und einer pakistanischen Familie führen. Mit Hilfe einer funktionellen Charakterisierung auf RNA-Ebene (Spleiß-Assay und RT-qPCR) konnte der Funktionsverlust beider Varianten bestätigt werden. Der zweite Abschnitt umfasst eine deutsche Familie mit sieben von einer HS betroffenen Familienmitgliedern, in der eine heterozygote missense Variante in MYO3A identifiziert wurde. In der vorliegenden Arbeit konnte somit die erste autosomal dominante Variante in einer europäischen Familie mit einer bilingualen, sensorineuralen Hochtonschwerhörigkeit beschrieben werden und der dominante Charakter von MYO3A bestätigt werden. Im dritten Abschnitt konnten die krankheitsverursachenden Varianten in 13 Familien aus einer Kohorte mit 21 pakistanischen Familien mit einer syndromalen und nicht-syndromalen HS ausfindig gemacht werden. Hierbei wurden sowohl bekannte, als auch bisher nicht beschriebene Varianten detektiert. Die Aufklärungsrate innerhalb dieser Kohorte betrug 61,9% und es konnte somit das Spektrum syndromaler und nicht-syndromaler HS erweitert werden. Der letzte Abschnitt dieser Arbeit beschreibt eine iranische Familie mit einer milden HS und milden Intelligenzminderung, in der eine homozygote missense Variante im Kandidatengen DBN1 ausfindig gemacht wurde. Um die Funktion und die Auswirkungen eines potentiellen Verlusts des codierten Proteins Drebrin zu untersuchen, wurden immunhistochemische Färbungen und auditorische Messungen an Dbn1 Knockout (KO)-Mäusen durchgeführt. Hierbei konnte eine Expression innerhalb der Nervenfasern, die innere Haarzellen innervieren, nachgewiesen werden. Eine leicht verlängerte Latenz für die ABR-Welle IV in KO-Mäusen im Vergleich zum Wildtyp ergab den Hinweis auf einen Defekt innerhalb des zentralen auditorischen Signalwegs, der möglicherweise mit einer Sprachverarbeitungsstörung im Menschen korreliert.
Background
Haemophilus influenzae (Hi) is a Gram-negative bacterium that may cause sepsis or meningitis, treatment of which mainly includes β-lactam antibiotics. Since 2019 EUCAST breakpoints for piperacillin/tazobactam have been available. Little is known about the prevalence and mechanisms of piperacillin/tazobactam resistance in Hi.
Objectives
To provide reliable prevalence data for piperacillin/tazobactam resistance in Hi in Germany, to evaluate different antibiotic susceptibility testing methods and to examine possible resistance mechanisms.
Methods
According to EUCAST breakpoints, the MIC for piperacillin/tazobactam resistance is >0.25 mg/L. All invasive Hi in Germany from 2019 were examined by gradient agar diffusion (GAD) for piperacillin/tazobactam susceptibility. Piperacillin/tazobactam broth microdilution (BMD), piperacillin GAD on tazobactam-containing agar [piperacillin GAD on Mueller–Hinton agar with horse blood (MH-F)/tazobactam) and piperacillin/tazobactam agar dilution (AD) were used for confirmation. Phenotypic testing was complemented by ftsI sequencing.
Results
Piperacillin/tazobactam GAD resulted in 2.9% (21/726) resistant Hi. BMD did not confirm piperacillin/tazobactam resistance. Two strains were found resistant by AD, of which one was also resistant using piperacillin GAD on MH-F/tazobactam. Overall, we found two strains with a piperacillin/tazobactam MIC >0.25 mg/L in at least two different tests (0.3%). Both were β-lactamase-producing amoxicillin/clavulanate-resistant with PBP3 mutations characterized as group III-like+. Relevant PBP3 mutations occurred in six strains without phenotypic piperacillin/tazobactam resistance. These mutations suggest a reduced efficacy of β-lactam antibiotics in these isolates.
Conclusions
Piperacillin/tazobactam resistance prevalence in invasive Hi is low in Germany. Reduced susceptibility was correlated with PBP3 mutations, in particular with group III mutations.
According to the WHO, foodborne derived enteric infections are a global disease burden and often manifest in diseases that can potentially reach life threatening levels, especially in developing countries. These diseases are caused by a variety of enteric pathogens and affect the gastrointestinal tract, from the gastric to the intestinal to the rectal tissue. Although the complex mucosal structure of these organs is usually well prepared to defend the body against harmful agents, specialised pathogens such as Salmonella enterica can overcome the intestinal defence mechanism. After ingestion, Salmonella are capable of colonising the gut and establishing their proliferative niche, thereby leading to inflammatory processes and tissue damage of the host epithelium. In order to understand these processes, the scientific community in the last decades mostly used cell line based in vitro approaches or in vivo animal studies. Although these approaches provide fundamental insights into the interactions between bacteria and host cells, they have limited applicability to human pathology. Therefore, tissue engineered primary based approaches are important for modern infection research. They exhibit the human complexity better than traditional cell lines and can mimic human-obligate processes in contrast to animal studies.
Therefore, in this study a tissue engineered human primary model of the small intestinal epithelium was established for the application of enteric infection research with the exemplary pathogen Salmonella Typhimurium.
To this purpose, adult stem cell derived intestinal organoids were used as a primary human cell source to generate monolayers on biological or synthetic scaffolds in a Transwell®-like setting. These tissue models of the intestinal epithelium were examined for their comparability to the native tissue in terms of morphology, morphometry and barrier function. Further, the gene expression profiles of organotypical mucins, tight junction-associated proteins and claudins were investigated. Overall, the biological scaffold-based tissue models showed higher similarity to the native tissue - among others in morphometry and polarisation. Therefore, these models were further characterised on cellular and structural level. Ultrastructural analysis demonstrated the establishment of characteristic microvilli and tight-junction connections between individual epithelial cells. Furthermore, the expression pattern of typical intestinal epithelial protein was addressed and showed in vivo-like localisation. Interested in the cell type composition, single cell transcriptomic profiling revealed distinct cell types including proliferative cells and stem cells, progenitors, cellular entities of the absorptive lineage, Enterocytes and Microfold-like cells. Cells of the secretory lineage were also annotated, but without distinct canonical gene expression patterns. With the organotypical polarisation, protein expression, structural features and the heterogeneous cell composition including the rare Microfold-like cells, the biological scaffold-based tissue model of the intestinal epithelium demonstrates key requisites needed for infection studies with Salmonella.
In a second part of this study, a suitable infection protocol of the epithelial tissue model with Salmonella Typhimurium was established, followed by the examination of key features of the infection process. Salmonella adhered to the epithelial microvilli and induced typical membrane ruffling during invasion; interestingly the individual steps of invasion could be observed. After invasion, time course analysis showed that Salmonella resided and proliferated intracellularly, while simultaneously migrating from the apical to the basolateral side of the infected cell. Furthermore, the bacterial morphology changed to a filamentous phenotype; especially when the models have been analysed at late time points after infection. The epithelial cells on the other side released the cytokines Interleukin 8 and Tumour Necrosis Factor α upon bacterial infection in a time-dependent manner. Taken together, Salmonella infection of the intestinal epithelial tissue model recapitulates important steps of the infection process as described in the literature, and hence demonstrates a valid in vitro platform for the investigation of the Salmonella infection process in the human context.
During the infection process, intracellular Salmonella populations varied in their bacterial number, which could be attributed to increased intracellular proliferation and demonstrated thereby a heterogeneous behaviour of Salmonella in individual cells. Furthermore, by the application of single cell transcriptomic profiling, the upregulation of Olfactomedin-4 (OLFM4) gene expression was detected; OLFM4 is a protein involved in various functions including cell immunity as well as proliferating signalling pathways and is often used as intestinal stem cell marker. This OLFM4 upregulation was time-dependent, restricted to Salmonella infected cells and seemed to increase with bacterial mass. Investigating the OLFM4 regulatory mechanism, nuclear factor κB induced upregulation could be excluded, whereas inhibition of the Notch signalling led to a decrease of OLFM4 gene and protein expression. Furthermore, Notch inhibition resulted in decreased filamentous Salmonella formation. Taken together, by the use of the introduced primary epithelial tissue model, a heterogeneous intracellular bacterial behaviour was observed and a so far overlooked host cell response – the expression of OLFM4 by individual infected cells – could be identified; although Salmonella Typhimurium is one of the best-studied enteric pathogenic bacteria. This proves the applicability of the introduced tissue model in enteric infection research as well as the importance of new approaches in order to decipher host-pathogen interactions with higher relevance to the host.
Since the prediction of the quantum spin Hall effect in graphene by Kane and Mele, \(Z_2\) topology in hexagonal monolayers is indissociably linked to high-symmetric honeycomb lattices. This thesis breaks with this paradigm by focusing on topological phases in the fundamental two-dimensional hexagonal crystal, the triangular lattice. In contrast to Kane-Mele-type systems, electrons on the triangular lattice profit from a sizable, since local, spin-orbit coupling (SOC) and feature a non-trivial ground state only in the presence of inversion symmetry breaking. This tends to displace the valence charge form the atomic position. Therefore, all non-trivial phases are real-space obstructed. Inspired by the contemporary conception of topological classification of electronic systems, a comprehensive lattice and band symmetry analysis of insulating phases of a \(p\)-shell on the triangular lattice is presented. This reveals not only the mechanism at the origin of band topology, the competition of SOC and symmetry breaking, but sheds also light on the electric polarization arising from a displacement of the valence charge centers from the nuclei, i. e., real-space obstruction. In particular, the competition of SOC versus horizontal and vertical reflection symmetry breaking gives rise to four topologically distinct insulating phases: two kinds of quantum spin Hall insulators (QSHI), an atomic insulator and a real-space obstructed higher-order topological insulator. The theoretical analysis is complemented with state-of-the-art first principles calculations and experiments on trigonal monolayer adsorbate systems. This comprises the recently discovered triangular QSHI indenene, formed by In atoms, and focuses on its topological classification and real-space obstruction. The analysis reveals Kane-Mele-type valence bands which profit from the atomic SOC of the triangular lattice. The realization of a HOTI is proposed by reducing SOC by considering lighter adsorbates. Further the orbital Rashba effect is analyzed in AgTe, a consequence of mirror symmetry breaking, the formation of local angular momentum polarization and SOC. As an outlook beyond topology, the Fermi surface and electronic susceptibility of Group V adsorbates on silicon carbide are investigated.
In summary, this thesis elucidates the interplay of symmetry breaking and SOC on the triangular lattice, which can promote non-trivial insulating phase.
Die Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung (ADHS) gehört weltweit zu den bedeutendsten psychiatrischen Erkrankungen des Kinder- und Jugendalters. Die Pathomechanismen sind aktuell noch nicht vollständig geklärt, wobei es deutliche Hinweise auf hirnorganische Veränderungen gibt. Die transkranielle Sonographie stellt eine nicht-invasive Methode dar, strukturelle Unterschiede tiefer Hirnstrukturen zu untersuchen. Bereits in vorangegangenen Studien konnte mit der Methode eine Veränderung der Echogenität der Substantia nigra (SN) bei Kindern mit ADHS im Vergleich zu gesunden Kontrollprobanden nachgewiesen werden. In dieser Studie sollen nun die möglichen physiologischen Hintergründe der erhöhten Echogenität der Substantia Nigra in Zusammenschau mit bildgebenden Verfahren betrachtet werden. Hierzu wurde in der vorliegenden multimodalen Studie bei 20 männlichen Kindern mit ADHS im Alter zwischen 8 und 12 Jahren eine transkranielle Ultraschalluntersuchung (TCS) zur Bestimmung der echogenen Fläche der Substantia Nigra sowie ein neuromelaninsensitives cMRT zur Bestimmung des neuromelaninassoziierten Volumens der SN, sowie des neuromelaninassoziierten Kontrastes SN/Cb durchgeführt. Als Kennwerte des peripheren Eisenhaushalts wurden die Konzentrationen von Eisen, Ferritin und Transferrin im Blut bestimmt. In die Auswertung gingen außerdem die Stärke der ADHS-Symptomatik (Strength and Difficulties Questionaire, SDQ; Fremdbeurteilungsbogen bei ADHS, FBB-ADHS), die kognitive Begabung (über CFT-20-R) und das Alter der Probanden ein. Psychiatrische Komorbidität wurde mit Hilfe der Child Behaviour Checklist (CBCL) erhoben.
Das Leitmotiv „Nachhaltigkeit“ durchdringt mit ungebrochener Dynamik das Recht in seiner nationalen, supranationalen und internationalen Ausgestaltung und erweist sich als bestimmendes Momentum der Rechtsetzung. So auch im Rahmen der europäischen Regulierung zu Sustainable Finance, welche die klimaneutrale Transformation der Realwirtschaft über das Vehikel nachhaltiger Finanzprodukte zum Ziel hat. Dieser Aufsatz untersucht nach einem kurzen Abriss zur Zielsetzung und Ausgangslage des Rechtsrahmens dessen einzelne Maßnahmen, namentlich die Offenlegungs-Verordnung (VO), Taxonomie-VO, Benchmark-VO und Green-Bond-VO, unter Berücksichtigung der regulatorischen Leitprinzipien und der rechtlichen Ausgestaltung des Nachhaltigkeitsbegriffs im Verhältnis zu ESG und Sustainable Finance. Dabei wird sich zeigen, dass zwar die Summe der Rechtsakte ein substanzielles Umdenken auf dem Kapitalmarkt zu etablieren vermag, die zugrunde liegende rechtliche Ausgestaltung von „Nachhaltigkeit“ jedoch weder trennscharf noch kongruent gelingt. Alternativ hierzu wird ein kontextabhängiger Definitionsansatz präsentiert, um der überbordenden Regulierung vermöge eines genuin europäischen Nachhaltigkeitskontext entgegenzuwirken und den mit nachhaltigen Finanzprodukten verbundenen Erwartungslagen besser Rechnung zu tragen.
Die Erkrankung MC zählt zusammen mit der Colitis Ulcerosa zu den CED. In Deutschland liegt die Prävalenz bei ca. 100-200 Personen pro 100000 Einwohner und steigt stetig. Überwiegend sind Menschen im jungen Erwachsenenalter betroffen, die fest im Berufsleben stehen. Die Erkrankung führt im Verlauf immer wieder zu Arbeitsausfällen und verursacht neben den gesundheitlichen Kosten für Medikamente und Therapie auch wirtschaftliche Ausfälle. Trotz der hohen sozioökonomischen Bedeutung und des Vorliegens gesicherter Erkenntnisse zu Risikofaktoren, anatomischen / histologischen Veränderungen, Symptomkomplexen und zahlreicher Hypothesen bezüglich der Entstehung, ist die Pathogenese nicht gänzlich verstanden. Ebenso komplex wie das Erkrankungsbild selbst ist der Prozess der Diagnosefindung. Ein Goldstandard ist nicht etabliert. Die Diagnose MC ist meist eine klinische, in Zusammenschau mit endoskopischen, histologischen, laborchemischen und radiologischen Befunden. In dieser Arbeit wurde die Versorgungsrealität der MC Erkrankten über die letzten 15 Jahre betrachtet. Es konnte ein Wandel in der chirurgischen Operations- und Anastomosentechnik gezeigt werden. Die Zukunft ist eine minimalinvasive und darmsparende Chirurgie. Im retrospektiven Vergleich der Therapiealgorithmen erfolgte nach damaliger S3-Leitlinie in der Mehrzahl der MC Fälle initial eine medikamentöse Therapie, alle untersuchten Fälle erhielten jedoch eine ICR. In der Subgruppenanalyse wurden Fälle mit isolierter Ileitis terminalis Crohn untersucht. Es konnte die Effektivität der chirurgischen Primärtherapie gegenüber einer medikamentösen Primärtherapie gezeigt werden, was die Daten der aktuellen Literatur stützt. So bestand ein Vorteil hinsichtlich des verringerten Bedarfs an einer medikamentösen Therapie im postoperativen Verlauf von zwei Jahren und bezüglich der rezidiv- und medikamentenfreien Zeit. Die Ergebnisse zeigten zudem, dass eine präventive, postoperative medikamentöse Therapie bei präoperativ vorliegenden Risikofaktoren für ein klinisches Rezidiv nicht zwingend notwendig ist und überdacht werden sollte. Diese Arbeit konnte den Stellenwert der Chirurgie als wichtige Säule der Therapie bei isolierter Ileitis terminalis Crohn untermauern.
Neisseria meningitidis (the meningococcus) is one of the major causes of bacterial meningitis, a life-threatening inflammation of the meninges. Traversal of the meningeal blood-cerebrospinal fluid barrier (mBCSFB), which is composed of highly specialized brain endothelial cells (BECs), and subsequent interaction with leptomeningeal cells (LMCs) are critical for disease progression. Due to the human-exclusive tropism of N. meningitidis, research on this complex host-pathogen interaction is mostly limited to in vitro studies. Previous studies have primarily used peripheral or immortalized BECs alone, which do not retain relevant barrier phenotypes in culture. To study meningococcal interaction with the mBCSFB in a physiologically more accurate context, BEC-LMC co-culture models were developed in this project using BEC-like cells derived from induced pluripotent stem cells (iBECs) or hCMEC/D3 cells in combination with LMCs derived from tumor biopsies.
Distinct BEC and LMC layers as well as characteristic expression of cellular markers were observed using transmission electron microscopy (TEM) and immunofluorescence staining. Clear junctional expression of brain endothelial tight and adherens junction proteins was detected in the iBEC layer. LMC co-culture increased iBEC barrier tightness and stability over a period of seven days, as determined by sodium fluorescein (NaF) permeability and transendothelial electrical resistance (TEER). Infection experiments demonstrated comparable meningococcal adhesion and invasion of the BEC layer in all models tested, consistent with previously published data. While only few bacteria crossed the iBEC-LMC barrier initially, transmigration rates increased substantially over 24 hours, despite constant high TEER. After 24 hours of infection, deterioration of the barrier properties was observed including loss of TEER and altered expression of tight and adherens junction components. Reduced mRNA levels of ZO-1, claudin-5, and VE-cadherin were detected in BECs from all models. qPCR and siRNA knockdown data suggested that transcriptional downregulation of these genes was potentially but not solely mediated by Snail1. Immunofluorescence staining showed reduced junctional coverage of occludin, indicating N. meningitidis-induced post-transcriptional modulation of this protein, as previous studies have suggested. Together, these results suggest a potential combination of transcellular and paracellular meningococcal traversal of the mBCSFB, with the more accessible paracellular route becoming available upon barrier disruption after prolonged N. meningitidis infection. Finally, N. meningitidis induced cellular expression of pro-inflammatory cytokines and chemokines such as IL-8 in all mBCSFB models. Overall, the work described in this thesis highlights the usefulness of advanced in vitro models of the mBCSFB that mimic native physiology and exhibit relevant barrier properties to study infection with meningeal pathogens such as N. meningitidis.
Die idiopathische Lungenfibrose (IPF) stellt eine chronische Krankheit mit einer schlechten Prognose dar. Die Erkrankung zeichnet sich durch ein dysfunktionales Alveolarepithel, die Formation von α-smooth muscle actin (α-SMA)-positiven Myofibroblasten, eine starke Kollagendeposition sowie eine fehlgeleitete Inflammation aus. In der Vermittlung dieser pro-fibrotischen Effekte spielt das Zytokin transforming growth factor β (TGF-β) eine Schlüsselrolle. Aufgrund des tödlichen Verlaufs der IPF und der limitierten Therapieoptionen ist die Entdeckung neuer Behandlungsansätze erforderlich.
Der NO/cGMP-Signalweg ist in der Modulation grundlegender physiologischer Vorgänge wie der Blutdruckregulation und der Peristaltik involviert. Hierbei spielt die NO-sensitive Guanylyl-Cyclase (NO-GC) als NO-Rezeptor eine fundamentale Rolle. In der Lunge wird die NO-GC in glatten Muskelzellen und Perizyten exprimiert. Während das Enzym in glatten Muskelzellen die Relaxation der glatten Muskulatur vermittelt, reguliert die NO-GC in Perizyten die Angiogenese, die Kapillardurchlässigkeit und den Blutfluss. Neben den physiologischen Aufgaben wurden anti-fibrotische sowie anti-inflammatorische Effekte der NO-GC in Herz, Leber, Niere und Haut beschrieben.
Daher wurde im Rahmen dieser Arbeit die NO-GC auf eine anti-fibrotische und anti-inflammatorische Bedeutung in der Lungenfibrose der Maus überprüft. Hierzu wurden Wildtyp- (WT) und globale NO-GC-Knockout-Mäuse (GCKO) untersucht. Die Fibrose wurde durch einmalige, orotracheale Bleomycin-Gabe induziert und zu unterschiedlichen Zeitpunkten (Tag 7 und 21) untersucht. Unbehandelte (Tag 0) Tiere dienten als Kontrolle. Im ersten Teil dieser Arbeit wurde die NO-GC auf eine anti-fibrotische Wirkung untersucht. Mittels Immunfluoreszenz wurde das Verhalten der α-SMA-positiven Myofibroblasten in den platelet-derived growth factor receptor β (PDGFRβ)-positiven fibrotischen Regionen untersucht. Der Kollagengehalt wurde mithilfe eines Hydroxyprolin-Kollagenassays ermittelt. Die untersuchten Fibrose-Kriterien waren in beiden Genotypen an Tag 21 stärker ausgeprägt als an Tag 7. An Tag 21 konnten im GCKO mehr α-SMA-positive Myofibroblasten, ausgeprägtere PDGFRβ-positive fibrotische Areale und ein höherer Kollagengehalt als im WT festgestellt werden. Zudem zeigten die GCKO-Tiere ein schlechteres Überleben als WT-Mäuse. Diese Ergebnisse wiesen auf eine überschießende fibrotische Antwort im GCKO und somit auf eine anti-fibrotische Wirkung der NO-GC in der Bleomycin-induzierten Lungenfibrose hin. Dass an Tag 21 die Fibrose im GCKO stärker ausfiel als im WT, konnte mit dem signifikant höheren TGF-β-Gehalt in der bronchoalveolären Lavageflüssigkeit (BALF) im GCKO erklärt werden. Das Fehlen der NO-GC im GCKO könnte zu einem Wegfall der Inhibierung der TGF-β-vermittelten, pro-fibrotischen Effekte durch die NO-GC führen. Weitere Studien sind erforderlich, um die Hypothese zu belegen und zugrundeliegende Mechanismen aufzuklären.
Die de novo Entstehung von Myofibroblasten, die maßgeblich an der Kollagensynthese beteiligt sind, stellt ein entscheidendes Fibrose-Merkmal dar. Umso bedeutender ist die Identifikation zweier Myofibroblasten-Subtypen, die sich in Lokalisation, NO-GC-Expression und Herkunft unterscheiden: (1) interstitielle, NO-GC-positive Myofibroblasten, die von Perizyten abstammen und Kollagen Typ I produzieren, und (2) intra-alveoläre, NO-GC-negative Myofibroblasten, deren Ursprung noch nicht abschließend geklärt ist. Die Anwesenheit beider Myofibroblasten-Typen konnte zu beiden untersuchten Zeitpunkten nach Bleomycin-Gabe bestätigt werden. Die NO-GC-Expression der Alveolarwand-ständigen Myofibroblasten, deren Abstammung von NO-GC-positiven Perizyten sowie deren dauerhafte Präsenz sprechen für eine relevante Rolle der NO-GC in der murinen Lungenfibrose. In weiteren Untersuchungen müssen die exakten Funktionen und spezifische Marker der Myofibroblasten-Subtypen identifiziert werden.
Im zweiten Teil dieser Arbeit wurde die NO-GC auf anti-inflammatorische Effekte in der Bleomycin-induzierten Lungenfibrose untersucht. Mittels HE-Färbung und Immunfluoreszenz wurden lymphozytäre Infiltrate an Tag 21 im GCKO festgestellt, was auf einen modulatorischen Einfluss der NO-GC auf das Immunsystem hindeutete. An Tag 21 wurden in der BALF von GCKO-Tieren signifikant mehr Gesamtimmunzellen, Lymphozyten und neutrophile Granulozyten als im WT gezählt, was auf eine starke Einwanderung von Immunzellen und somit auf eine ausgeprägte Entzündung in GCKO-Lungen hinwies. Folglich könnte die NO-GC eine anti-inflammatorische Rolle über die Regulation der Immigration von Immunzellen in der Bleomycin-induzierten Lungenfibrose spielen. In der Literatur werden pro- und anti-fibrotische Effekte der Immunzellen in der murinen Lungenfibrose diskutiert. Durch Korrelationsanalysen wurde ein positiver Zusammenhang zwischen der Gesamtimmunzellzahl und der TGF-β-Konzentration an Tag 21 festgestellt. In verschiedenen Studien wurde ein pro-fibrotischer Einfluss der Immunzellen über die Aktivierung/Sekretion von TGF-β beschrieben. Die Abwesenheit der NO-GC im GCKO könnte also über die verstärkte Immigration von Immunzellen in einem erhöhten TGF-β-Gehalt resultieren und so zu einer überschießenden fibrotischen Reaktion an Tag 21 führen. Auf welche Weise die NO-GC die Einwanderung der Immunzellen in der Bleomycin-induzierten Lungenfibrose beeinflusst, muss in weiteren Studien untersucht werden. Zusammenfassend deuten die Daten dieser Arbeit auf eine anti-inflammatorische und anti-fibrotische Rolle der NO-GC in der Lungenfibrose der Maus hin.
Bereits in Vorstudien konnte dargelegt werden, dass eine signifikante Korrelation zwischen der T-Zell-Zytokin-Antwort und der berufs- bzw. umweltbedingten Schimmelpilzbelastung besteht. Ziel der vorliegenden Studie war, eine mögliche Kombination von Biomarkern ausfindig zu machen, die veränderte T-Zell-Antworten auf A. fumigatus- Antigene bei beruflich Exponierten im Vergleich zu Kontrollprobanden/-innen vorhersagen kann. Um geeignete Marker für das Bio-Monitoring zu finden, wurden zur T-Zell-Aktivierung ein myzeliales A. fumigatus - Lysat und 12 proteinogene Antigene in ELISpot-Versuchen für die Signaturzytokine IFN-γ (TH1), IL-5 (TH2) und IL-17A (TH17) der Haupt-TH-Subpopulationen getestet.
Es zeigten sich bei den Biolandwirten/-innen erwartungsgemäß erhöhte TH1- und TH2-Antworten auf die Mehrzahl der verwendeten spezifischen A. fumigatus-Antigene, die möglicherweise eine Schimmelpilzbelastung serologisch nachweisbar machen. Insbesondere die spezifischen A. fumigatus-Antigene Aspf22, CatB und CipC konnten eine Trennschärfe zwischen den beiden Kohorten hinsichtlich ihrer IFN-γ- und IL-5-Zytokinantwort erzielen. Unterschiede in der TH17-Antwort aufgrund chronischer beruflicher Sporenbelastung ohne Krankheitskorrelat konnten nicht explizit festgestellt werden. Weiterhin ergab sich, dass erhöhte TH2-Immunreaktionen, sofern sie mit einer adäquaten TH1-gerichteten Immunantwort einhergehen und damit eine ausgeglichene TH2/TH1-Balance besteht, nicht zwangsläufig zu Hypersensitivitätserkrankungen führen. Im Vergleich zu Langzeitexponierten wurden teilweise überlappende TH-Zellfrequenzen bei beruflich exponierten Biolandwirten/-innen ermittelt. Welche entscheidende Rolle Treg-Zellen bei der Eindämmung überschießender Immunantworten einnehmen, kann hieraus erahnt werden.
Störungen der Ionen- und Blutgas Homöostase mit Verschiebungen von Na+ und K+ in der regionalen Hypoxie sind ein Kennzeichen der experimentellen zerebralen Ischämie, wurden aber in ihrer Bedeutung für Schlaganfallpatienten noch nicht hinreichend untersucht. Wir berichten über eine prospektive, humane Querschnittsstudie an 366 Schlaganfallpatienten, die mit einer endovaskulären Rekanalisation bei einem akuten LVO der vorderen Zirkulation zwischen dem 18.Dezember 2018 und dem 31.August 2020 behandelt wurden. Im Rahmen der vorliegenden Dissertationsarbeit wurden intraprozedural arterielle Blutgasproben (1ml) aus dem lokal ischämischen Kollateralkreislauf und der intraindividuellen systemischen Referenzlokalisation in 51 Patienten gewonnen. Die Probengewinnung mit Hilfe eines Mikrokatheters erfolgte nach einem bereits veröffentlichten Protokoll.
Diese Arbeit weist in der Perakutphase eines Großgefäßverschlusses signifikant nach, dass der lokal ischämische paO2 (-4,29%, paO2ischämisch=185,3 mmHg vs. paO2systemisch=193,6mmHg; p=0,035) und die Konzentration von K+ (-5,49%, K+ischämisch=3,44mmol/L vs. K+systemisch=3,64mmol/L; p=0,0081) signifikant reduziert war. Wir beobachteten, dass der Na+:K+-Quotient in der Kollateralzirkulation (+3,29%; Na+:K+-Quotientischämisch=41,74 vs. Na+:K+-Quotientsystemisch=40,38; p=0,0048) im Vergleich zur systemischen Zirkulation signifikant erhöht war, während die Na+-Konzentration signifikant positiv mit einer Zunahme des Infarktausmaßes assoziiert war (r=0,42, p=0,0033). Wir fanden eine alkaline Tendenz des zerebralen pH (+0,14%, pHischämisch=7,38 vs. pHsystemisch=7,37, p=0,0019), mit einer zeitabhängigen Verschiebung in den azidotischen Bereich (r=-0,36, p=0,0549).
Schlussfolgernd deuten unsere Ergebnisse darauf hin, dass die durch den Schlaganfall verursachten Veränderungen der zerebralen Sauerstoffversorgung, der Ionenzusammensetzung und des Säure-Basen-Gleichgewichts dynamisch auftreten, während der okklusiven Ischämie fortschreiten und mit der akuten Gewebeschädigung im Zusammenhang stehen. Wünschenswert sind weitere prospektive Studien, um die Ergebnisse valide zu reproduzieren.
Die vorliegende Arbeit widmet sich den Reaktionsmöglichkeiten, welche die Rechtsordnung der Europäischen Union ihren Organen zur Sicherung der in Artikel 2 EUV kodifizierten Werte zur Verfügung stellt. Die Europäische Union wird hierbei in ihrer Eigenschaft als Wertegemeinschaft, die sich insbesondere auf demokratischen, rechtsstaatlichen und menschenrechtlichen Zielen und Vorstellungen gründet, beleuchtet. Neben der Analyse der Werteklausel nach Artikel 2 EUV setzt sich die Dissertation im Kern sowohl mit den präventiven als auch repressiven Instrumenten des unionalen Primär- und Sekundärrechts zur Sicherung des Wertekanons auseinander. Im Wege eines systematischen Vergleichs erfolgt abschließend eine Bewertung der verschiedenen Handlungsmechanismen.
Die im Rahmen der Arbeit erzielten Ergebnisse liefern neue Erkenntnisse über einen neuen Sternzellsubtyp der murinen Leber. Bei Gewebeverletzung differenzieren Sternzellen im Allgemeinen zu Myofibroblasten, welche Extrazellulärmatrix produzieren. Des Weiteren sind Sternzellen die Perizyten der Leber und spielen eine Rolle in der Angiogenese und Gefäßremodellierung.
Der in präliminären Untersuchungen identifizierte Sternzellsubtyp zeichnet sich durch die Expression von tdTomato in Abhängigkeit des SMMHC-Promotors aus (SMMHC/tdTomato\(^+\) Sternzellen). In dieser Arbeit wurden SMMHC/tdTomato\(^+\) Sternzellen immunhistochemisch unter physiologischen und fibrotischen Bedingungen untersucht.
Mit Hilfe von Lineage Tracing konnte zunächst die Zellmauserung der SMMHC/tdTomato\(^+\) Sternzellen gezeigt werden. Durch Leberzonen-spezifische Marker wurde daraufhin nachgewiesen, dass SMMHC/tdTomato\(^+\) Sternzellen in Zone 1 des Leberazinus lokalisiert sind, weswegen diese Zellen im Weiteren „Zone 1-HSC“ genannt wurden. Als potenzielle Progenitorzellnische der Zone 1-HSC wurde das Portalfeld eingegrenzt.
Außerdem wurde die Funktion der Zone 1-HSC in der CCl\(_4\)-induzierten Leberfibrose untersucht. Es stellte sich heraus, dass Zone 1-HSC bereits früh in der Fibrose die Zonierung verlieren und diese auch nach Regenerationszeit nicht wiederhergestellt wird. Es wurde nachgewiesen, dass Zone 1-HSC nicht zu Myofibroblasten differenzieren. Stattdessen spielen Zone 1-HSC möglicherweise eine Rolle in der sinusoidalen Kapillarisierung in Folge einer CCl\(_4\)-induzierten Fibrose.
Zweck: Obwohl Bruxismus im Wesentlichen als Verhalten mit multifaktorieller Genese gilt, konnten bisher nicht eindeutig die damit assoziierten Komorbiditäten aufgeklärt werden. Die Zielsetzung war, anamnestische und psychosoziale Unterschiede zwischen Proband(inn)en mit und ohne möglichem bzw. definitivem Bruxismus zu ermitteln. Darüber hinaus sollte die Übereinstimmung verschiedener Instrumente zur Bruxismus-Diagnostik und der Effekt von zwei Interventionen (bedingte elektrische Stimulation (CES) und Kautraining) analysiert werden.
Methoden: In dieser klinischen, explorativen Studie wurden 76 Proband(inn)en untersucht. Die Proband(inn)en wurden in die drei Gruppen Kontrollgruppe, aktive Interventionsgruppe und Rehabite Interventionsgruppe eingeteilt. Die Kontrollgruppe trug ein portables EMG-Gerät (GrindCare) jede Nacht über einen Beobachtungszeitraum von 5 Wochen inaktiv. Die aktive Interventionsgruppe trug es die erste Woche inaktiv, dann 2 Wochen aktiv mit CES und anschließend erneut 2 Wochen inaktiv. Die RehaBite Interventionsgruppe verwendete das GrindCare eine Woche inaktiv, darauf folgte ein zweiwöchiges Kautraining mit einer Bissgabel namens RehaBite aber ohne EMG-Begleitung und die letzten zwei Wochen verliefen ohne Rehabite und GrindCare. Zu Beginn und am Ende des Beobachtungszeitraums füllten alle drei Gruppen die gleichen Fragebögen, u.a. die Oral Behavior Checklist (OBC) und verschiedene Fragebögen zu körperlichen und psychologischen Parametern, aus. Das GrindCare misst die Episoden und ermöglicht damit die Diagnose eines definitiven Schafbruxismus (SB).
Ergebnisse: Es existierten signifikante Unterschiede zwischen Proband(inn)en mit und ohne Bruxismus (möglicher und definitiver SB, möglicher Wachbruxismus (WB), möglicher kombinierter SB und WB) in diversen anamnestischen und psychosozialen Parametern. Außerdem bestand ein signifikanter Zusammenhang zwischen erhöhter Kieferaktivität (diagnostiziert mittels OBC) und SB- sowie WB-Selbstangabe sowie zwischen den Selbstangaben von SB und WB untereinander, nicht jedoch zwischen Fragebögen und apparativer Diagnostik. Die CES bewirkte keine Reduktion der Episoden, dafür verbesserten sich jedoch einzelne körperliche und psychosoziale Parameter in der aktiven bzw. in der Rehabite Gruppe im Laufe des Beobachtungszeitraums.
Fazit: Personen mit und ohne möglichem bzw. definitivem Bruxismus unterschieden sich in verschiedenen anamnestischen, körperlichen und psychosozialen Eigenschaften voneinander. Außerdem bestehen signifikante Korrelationen zwischen SB und WB laut Selbstangabe, nicht jedoch bezüglich der apparativen Bruxismus-Diagnostik mit dem GrindCare. Während die Episoden nicht durch die CES gesenkt wurden, verringerten sich -eventuell durch RehaBite bzw. CES bedingt- bestimmte Beschwerden. Weiterer Forschungsbedarf besteht, um auf der Basis größerer Stichproben die gefundenen Auffälligkeiten statistisch abzusichern.
Der Glycin-Rezeptor ist Teil der inhibitorischen liganden-gesteuerten Ionenkanäle im ZNS und wird am stärksten im adulten Rückenmark sowie im Hirnstamm exprimiert. In der Nerv-Muskel-Synapse sind GlyR für die rekurrente Hemmung der Motoneuronen wichtig und steuern das Gleichgewicht zwischen Erregung und Hemmung der Muskelzellen. Für die glycinerge Neurotransmission sind neben den präsynaptischen GlyR 𝛼1 insbesondere postsynaptische GlyR 𝛼1/𝛽 verantwortlich. Durch Mutationen des GlyR entsteht das Erkrankungsbild der Hyperekplexie mit übersteigerter Schreckhaftigkeit, Muskelsteifheit und Apnoe. Hauptursächlich dafür sind Mutationen im GLRA1-Gen. Die shaky Maus stellt ein gutes Modell zur Erforschung dieser seltenen Erkrankung dar.
Die shaky Missense-Mutation Q177K in der extrazellulären 𝛽8-𝛽9 Schleife der Glycin- Rezeptor-𝛼1-Untereinheit zeigte strukturell ein gestörtes Wasserstoffbrückennetzwerk. Funktionell konnten eingeschränkt leitfähige Ionenkanäle identifiziert werden. Der letale Phänotyp äußert sich beim homozygoten shaky Tier durch Schrecksymptome mit einem einhergehenden zunehmenden Gewichtsverlust. Die Quantifizierung der Oberflächenexpression deutete auf einen Verlust synaptischer GlyR 𝛼1/𝛽 hin. Aussagen bezüglich der GlyR-𝛽-Untereinheit, die Teil des synaptischen GlyR Komplexes ist, waren aufgrund fehlender stabiler Antikörper bisher nicht möglich. Das neuartige KI- Mausmodell Glrb eos exprimiert endogen fluoreszierende 𝛽 -Untereinheiten und ermöglicht damit erstmalig eine Betrachtung der GlyR- 𝛽-Expression in Tiermodellen der Startle Erkrankung.
Ziel dieser Arbeit war es, die Auswirkungen der shaky Mutation auf die Interaktion mit der 𝛽 -Untereinheit und Gephyrin zu erforschen. Dafür wurden Markerproteine der glycinergen Synapse in Rückenmarksneuronen der Kreuzung Glrb eos x Glra1 sh gefärbt und quantifiziert. Die durchgeführte Gewichtsbestimmung der Nachkommen im zeitlichen Verlauf zeigte keinen Einfluss der eingefügten mEos4b-Sequenz auf das Körpergewicht der Tiere und schließt damit funktionelle Einschränkungen bedingt durch die mEos4b-Sequenz aus. Zur Verstärkung des 𝛽 eos-Signals wurde ein Antikörper verwendet. Die Quantifizierung der GlyR- 𝛽- Untereinheit an Rückenmarksneuronen zeigte für homozygote shaky Tiere im Vergleich zum Wildtyp signifikant reduzierte 𝛽eos Oberflächenexpressionen in Gephyrin Clustern sowie signifikant erniedrigte Kolokalisationen von Gephyrin/𝛼1, 𝛽eos/𝛼1 und 𝛽eos/Gephyrin. Die mutierte GlyR-𝛼1- Untereinheit wurde hingegen vermehrt an der Oberfläche in shaky Tieren exprimiert. Die Ergebnisse der Rückenmarksschnitte unterstützen diese Befunde aus den Primärneuronen. Die Untersuchung der Präsynapse erbrachte für Glrb eos/eos x Glra1 sh/sh eine signifikant verminderte Synapsin und Synapsin/𝛼1 Expression.
Die Ergebnisse dieser Arbeit erweitern die Daten früherer Arbeiten zur shaky Maus und zeigen einen starken Verlust synaptischer GlyR 𝛼 1/ 𝛽 an der Oberfläche von Motoneuronen. Ein möglicher kompensatorischer Versuch durch erhöhte 𝛼1 Expression bleibt infolge der Funktionsbeeinträchtigung dieser mutierten GlyR- 𝛼 1 Rezeptoren erfolglos mit letalem Ausgang. In vorherigen Arbeiten wurde vermutet, dass die Mutation in der extrazellulären Bindungsstelle in der Lage ist, Konformationsänderungen in die TM3-TM4-Schleifenstruktur zu übertragen und dadurch die Gephyrin Bindung und synaptische Verankerung zu stören. Die Daten dieser Arbeit stützen diese Annahme und weisen darüber hinaus auf eine gestörte Rezeptorkomplexbindung hin. Die vorliegende Arbeit trägt somit zum besseren Verständnis der Startle Erkrankung auf synaptischer Ebene bei.
Sprachideologien als zugrundeliegende Annahmen über Sprachstrukturen und Sprachgebrauch schlagen sich häufig in metasprachlichen Diskursen und vor allem im laienlinguistischen Austausch über Sprache nieder. Die einschlägige Forschung zu derartigen Metasprachdiskursen hat sich bislang nur vereinzelt dem laienlinguistischen Austausch in den Sozialen Medien gewidmet.
Um Einblicke in laienlinguistische Metasprachdiskurse jüngerer Generationen zu geben, untersucht die vorliegende Studie die Beiträge sowie die dazugehörigen Kommentare zweier Instagram-Profile, die regelmäßig sprachliches Wissen vermitteln, hinsichtlich ihrer sprachideologischen Standpunkte. Hierzu wurde das stancetaking der Nutzenden untersucht, d.h. es wurde nachvollzogen, wie sich Sprachteilnehmende lexikalisch, grammatisch, aber auch im Rahmen multimodaler Möglichkeiten der Plattform (z.B. Farben, Emojis, Likes) zu Sprache und zu anderen Sprachteilnehmenden positionieren.
Es zeigt sich, dass vor allem sprachrichtigkeitsideologische Einstellungen in den untersuchten Beiträgen vorzufinden sind. Varianten werden hier explizit sprachlich als richtig oder falsch oder auch implizit mittels farblicher Markierungen gekennzeichnet. Standardideologische Annahmen kommen in den Beiträgen häufig in der Orientierung an kodifizierenden Institutionen und in der Abgrenzung zu einer Umgangssprache zum Tragen, wobei die Beiträge von einer Thematisierung des Standardbegriffs absehen. Vereinzelt werden die Sprachrichtigkeits- und die Standardideologie in den Kommentaren durch Nutzende in Frage gestellt; insbesondere, wenn diese sich als linguistisch Sachkundige positionieren.
Chondrozyten stellen die zelluläre Komponente von hyalinem Knorpel dar, der die Gelenkflächen diarthrotischer Gelenke bedeckt. Über die perizelluläre Matrix (PZM) sind sie mit der extrazellulären Matrix des Knorpelgewebes, die im Wesentlichen aus Wasser, Kollagen-Typ-II (Koll-II) und Glykosaminoglykan (GAG) gebildet wird, verbunden. Die PZM gilt als wichtiges modulatorisches und protektives Element in der Signal- und Mechanotransduktion sowie für die Homöostase innerhalb des Knorpelgewebes. Degenerative und inflammatorische Prozesse führen zu irreparablen Schäden der Gewebearchitektur und -funktionalität. Die Regenerative Medizin strebt den Ersatz destruierter Gelenkflächen durch mittels Tissue Engineering hergestellten Neoknorpel an. 3D-Bioprinting gilt hier als attraktive Methode, nimmt jedoch über Scherkräfte während des Druckvorgangs auch schädigenden Einfluss auf das Überleben oder die Funktionalität der Zellen.
Zielsetzung dieser Arbeit war es, den möglichen protektiven Einfluss der PZM während des Druckvorgangs zu untersuchen. Aus porcinem Frischknorpel isolierte Chondrozyten wurden nach cast bzw. 3D-Bioprinting in Agarose-Biotinte hinsichtlich ihres Überlebens und ihrer Syntheseleistung von knorpelspezifischem Koll-II und GAG untersucht. Chondrozyten ohne PZM wurden mit Chondrozyten verglichen, die nach enzymatischer Isolation noch perizellulär Kollagen-Typ-VI als Marker der PZM aufwiesen. Chondrozyten mit PZM zeigten allgemein eine stärkere Produktion von Koll-II als Chondrozyten ohne PZM. Nach 3D-Bioprinting konnte für Chondrozyten ohne PZM eine signifikant geringere Produktion von GAG nachgewiesen werden als in der cast-Vergleichsgruppe, während dies für Chondrozyten mit PZM nicht gezeigt werden konnte.
Der gezeigte protektive Einfluss der PZM gegenüber Scherkräften während des Druckvorgangs eröffnet neue Methoden für das Cartilage Tissue Engineering. Weitere Untersuchungen sind notwendig, um dies zu bestätigen und die Translation in die klinische Forschung ermöglichen.
Learning accompanies us throughout our lives, from early childhood education through
school, training and university to learning at work. However, much of what we learn is quickly
forgotten. The use of practice tests is a learning strategy that contributes to the acquisition of
sustainable knowledge, i.e. knowledge that is permanently available and can be retrieved when
it is needed. This dissertation first presents findings from previous research on testing in real
educational contexts and discusses theoretically why certain learner or situational
characteristics might influence the effectiveness of the testing effect. Furthermore, a cycle of
three experiments is presented, which were used to investigate whether the positive effect of
practice tests on retention (testing effect) depends on personal or situational characteristics and
also promotes the retention of lecture content that was not directly tested (transfer) in the context
of regular psychology lectures in teacher training courses. In an additional chapter, feedback
from students on the implementation of the study in the classroom context is examined in more
detail. Finally, the results of the three studies are discussed and placed in relation to the theories
presented. The central conclusion from the studies presented is that the testing effect appears to
be a very effective learning strategy that can be used effectively in university teaching and leads
to better learning outcomes regardless of learner characteristics. However, the practice tests
should cover the entire range of relevant content, as transfer effects to non-tested content are
not to be expected.
This paper examines the potential reinforcement of motivated beliefs when individuals with identical biases communicate. We propose a controlled online experiment that allows to manipulate belief biases and the communication environment. We find that communication, even among like-minded individuals, diminishes motivated beliefs if it takes place in an environment without previously declared external opinions. In the presence of external plural opinions, however, communication does not reduce but rather aggravates motivated beliefs. Our results indicate a potential drawback of the plurality of opinions - it may create communication environments wherein motivated beliefs not only persist but also become contagious within social networks.
Mit der EEG-Umlage wurden bis zum Jahr 2023 die Förderkosten für den Ökostromausbau auf die Stromverbraucher umgelegt. Ursprünglich knüpfte die Umlagepflicht an eine Stromlieferung an, weshalb der eigenerzeugte Strom nicht erfasst war. Das sog. Eigenstromprivileg wurde mit dem EEG 2014 grundsätzlich abgeschafft. Allerdings führten diverse Ausnahmetatbestände dazu, dass ein Großteil der Eigenverbrauchsmengen weiterhin privilegiert waren.
Die Arbeit untersucht, in welchem Maße dieser Flickenteppich an unterschiedlichen Regelungen auf die verfassungs- und europarechtlichen Rahmenbedingungen zurückzuführen ist. Neben einer systematischen Erfassung des einfachgesetzlichen Rechtsrahmens erfolgt eine Analyse, inwiefern das Verfassungs- und Europarecht den gesetzlichen Gestaltungsspielraum für staatlich veranlasste, jedoch privatrechtlich ausgestaltete Umlagesysteme einschränkt.

Bei nur einem Testverfahren blieben zahlreiche Betroffene unentdeckt. Möglicherweise ist das verschiedenen pathophysiologischen Subgruppen geschuldet.
Background
Healthcare workers and medical students faced new challenges during the COVID-19 pandemic. Processes within many hospitals were completely disrupted. In addition, the face to face teaching of medical students was drastically reduced. Those at risk of developing mental health problems appear to be younger health care workers and women.
Objective
To investigate potential COVID-19 pandemic-related gender differences in psychological distress among medical students and physicians in their first years of practice.
Design and setting
An anonymous survey was carried out online between December 1, 2021, and March 31, 2022, at the Mannheim Medical Faculty and the Würzburg Medical Faculty, Germany, after obtaining informed consent. Primary outcome measures were changes in anxiety and depression symptoms using the Hospital Anxiety and Depression Scale (HADS), and changes in participants' current quality of life using the WHO Quality of Life BREF.
Results
The results show wave-like courses for perceived anxiety and burden overlapping with the course of the COVID-19 incidence. In comparison to men, women showed a significant higher increase in HADS (p = 0.005) and a reduced life quality (p = 0.007) after COVID-19. Both sexes showed different frequencies of the factors influencing quality of life, with the presence of a previous mental illness and mean anxiety having a significant higher negative impact in women.
Conclusion
Future and young female physicians reported a disproportionate higher burden during COVID-19 compared to their male colleges. These observations suggest an increased need for support and prevention efforts especially in this vulnerable population.
In der vorliegenden Dissertation wurde erstmalig systematisch
untersucht, wie häufig und in welcher Ausprägung laryngeale Konstriktionsphänomene in vorsprachlichen Vokalisationen von Säuglingen mit orofazialen Spalten in den Lebensmonaten 4 bis 6 auftraten. Eine statistische Analyse zur zeitlichen Dauer der laryngealen Konstriktionen wurde ebenfalls vorgenommen. Dabei wurde der potenzielle Einfluss der Spaltausprägung, der kieferorthopädischen Frühbehandlung mit einer Gaumenplatte, des Alters sowie des biologischen Geschlechts berücksichtigt.
Die Analyse von 9.345 vorsprachlichen Vokalisationen von 23 Säuglingen mit einer orofazialen Spalte, die im Rahmen der kieferorthopädischen Frühbehandlung eine Gaumenplatte erhielten, erfolgte vor dem theoretischen Hintergrund des Modells des laryngealen Artikulators (LAM) (Esling, 2005). Das LAM postuliert, dass die Entwicklung artikulatorischer Fähigkeiten bereits mit Lautmodifikationen im Larynx beginnt, den laryngealen Konstriktionen. Je nach Ausprägungsgrad führen laryngeale Konstriktionen zu unvollständigen oder vollständigen Unterbrechungen des glottalen Luftstroms und damit zu rhythmisch segmentierten Phonationen. Für die Stichprobe konnte mit Hilfe der angewandten Methodik gezeigt werden, dass Säuglinge mit einer orofazialen Spalte bereits vor den bekannten supralaryngealen Artikulationsstörungen Auffälligkeiten in laryngeal erzeugten präartikulatorischen Funktionen zeigen.
Es konnte gezeigt werden, dass die Dysfunktion mit dem Spalttyp im direkten Zusammenhang steht, wobei Säuglinge mit der größten oronasalen Malformation, die Träger bilateraler Lippen-Kiefer-Gaumen-Segelspalten, am stärksten betroffen waren.
Das vermehrte Auftreten von laryngealen Konstriktionen wurde auf Substitutionseffekte zurückgeführt, die die eingeschränkte Erzeugung supralaryngealer Laute ausgleichen könnte.
Hinsichtlich der Untersuchung des Einflusses der Plattentherapie auf vorsprachliche Vokalisationen von Säuglingen mit orofazialen Spalten ließen sich erste Hinweise finden, dass die Gaumenplatte einen Effekt auf das artikulatorische Explorationsverhalten haben könnte.
Die geringe Variabilität der Konstriktionslängen deutet auf regelhafte Prozesse hin und stützt vorhergehende Studien.
Um die tatsächliche artikulatorische Entwicklung der Patientinnen und Patienten mit orofazialen Spalten prä- und postoperativ besser verstehen zu können, genügt der alleinige Blick auf die Konsonantenentwicklung nicht. Nur wenn diese gemeinsam mit den laryngealen Konstriktionsphänomenen als artikulatorische Vorstufen betrachtet werden, wird es möglich sein, in Zukunft diagnostische Risikomarker für die Sprech- und Sprachentwicklung bei Säuglingen zu identifizieren.
Most medicines are taken orally. To enter the systemic circulation, they dissolve in the intestinal fluid, cross the epithelial barrier, and pass through the liver. Intestinal absorption is driven by the unique features of the gastrointestinal tract, including the bile colloids formed in the lumen and the mucus layer covering the intestinal epithelium. Neglecting this multifaceted environment can lead to poor drug development decisions, especially for poorly water-soluble drugs that interact with bile and mucus. However, there is a lack of a rationale nexus of molecular interactions between oral medicines and gastrointestinal components with drug bioavailability. Against this background, this thesis aims to develop biopharmaceutical strategies to optimize the presentation of oral therapeutics to the intestinal epithelial barrier.
In Chapter 1, the dynamics of bile colloids upon solubilization of the poorly-water soluble drug Perphenazine was studied. Perphenazine impacted molecular arrangement, structure, binding thermodynamics, and induced a morphological transition from vesicles to worm-like micelles. Despite these dynamics, the bile colloids ensured stable relative amounts of free drug substance. The chapter was published in Langmuir.
Chapter 2 examined the impact of pharmaceutical polymeric excipients on bile-mediated drug solubilization. Perphenazine and Imatinib were introduced as model compounds interacting with bile, whereas Metoprolol did not. Some polymers altered the arrangement and geometry of bile colloids, thereby affecting the molecularly soluble amount of those drugs interacting with bile. These insights into the bile-drug-excipient interplay provide a blueprint to optimizing formulations leveraging bile solubilization. The chapter was published in Journal of Controlled Release.
Chapter 3 deals with the impact of bile on porcine intestinal mucus. Mucus exposed to bile solution changed transiently, it stiffened, and the overall diffusion rate increased. The bile-induced changes eased the transport of the bile-interacting drug substance Fluphenazine, whereas Metoprolol was unaffected. This dichotomous pattern was linked to bioavailability in rats and generalized based on two previously published data sets. The outcomes point to a bile-mucus interaction relevant to drug delivery. The chapter is submitted.
The Appendix provides a guide for biopharmaceutical characterization of drug substances by nuclear magnetic resonance spectroscopy aiming at establishing a predictive algorithm.
In summary, this thesis deciphers bile-driven mechanisms shaping intestinal drug absorption. Based on these molecular insights, pharmaceuticals can be developed along a biopharmaceutical optimization, ultimately leading to better oral drugs of tomorrow.
Drug Discovery based on Oxidative Stress and HDAC6 for Treatment of Neurodegenerative Diseases

Most antioxidants reported so far only achieved limited success in AD clinical trials. Growing evidences suggest that merely targeting oxidative stress will not be sufficient to fight AD. While multi-target directed ligands could synergistically modulate different steps in the neurodegenerative process, offering a promising potential for treatment of this complex disease.
Fifteen target compounds have been designed by merging melatonin and ferulic acid into the cap group of a tertiary amide HDAC6 inhibitor. Compound 10b was screened as the best hybrid molecule exhibit potent HDAC6 inhibition and potent antioxidant capacity. Compound 10b also alleviated LPS-induced microglia inflammation and led to a switch from neurotoxic M1 to the neuroprotective M2 microglial phenotype. Moreover, compound 10b show pronounced attenuation of spatial working memory and long-term memory damage in an in vivo AD mouse model. Compound 10b can be a potentially effective drug candidate for treatment of AD and its druggability worth to be further studied.
We have designed ten novel neuroprotectants by hybridizing with several common antioxidants, including ferulic acid, melatonin, lipoic acid, and trolox. The trolox hybrid compound exhibited the most potent neuroprotective effects in multiple neuroprotection assays. Besides, we identified the synergistic effects between trolox and vitamin K derivative, and our trolox hybrid compound showed comparable neuroprotection with the mixture of trolox and vitamin K derivative.
We have designed and synthesized 24 quinone derivatives based on five kinds of different quinones including ubiquinone, 2,3,5-trimethyl-1,4-benzoquinone, memoquin, thymoquinone, and anthraquinone. Trimethylbenzoquinone and thymoquinone derivatives showed more potent neuroprotection than other quinones in oxytosis assay. Therefore, trimethylbenzoquinone and thymoquinone derivatives can be used as lead compounds for further mechanism study and drug discovery for treatment of neurodegenerative disease.
We designed a series of photoswitchable HDAC inhibitors, which could be effective molecular tools due to the high spatial and temporal resolution. In total 23 target compounds were synthesized and photophysicochemically characterized. Azoquinoline-based compounds possess more thermally stable cis-isomers in buffer solution, which were further tested in enzyme-based HDAC inhibition assay. However, none of those tested compounds show significant differences in activities between trans-isomers and corresponding cis-isomers.

Background: That a differentiated treatment of subjects with low and high levels of disabling pain might be necessarily has only been suspected but not sufficiently confirmed so far. Furthermore, the effectiveness of extraoral therapy methods for TMD is still controversial in the literature. The present work could make an important contribution to this.
Objectives: Five systematic reviews with meta-analysis were conducted to investigate the efficacy of extraoral therapies (acupuncture, laser, medication, psychosocial interventions, and physiotherapy) in the treatment of TMD in relation to the degree of chronicity of pain.
Literature sources: With this objective, the databases Pubmed/MEDLINE, EMBASE, Cochrane Library, Livivo, OpenGrey, drks.de, Clinicaltrials.gov. were searched.
Criteria for the selection of suitable studies: Adults suffering from painful TMD and treated with either acupuncture, laser, medication, psychosocial interventions, or physiotherapy. The studies were then examined for evidence in the subjects' characteristics suggesting that they were suffering from chronic TMD in terms of pain dysfunction. These included a high score on the GCPS, resistance to undergone treatments, multilocular pain, depression, and regular use of pain medication. The effectiveness of the five interventions was then differentiated according to the suspected degree of chronicity. Effectiveness was assessed by the following outcomes: patient- related current pain intensity, MMO, pain on palpation, temporomandibular joint sounds, depression, and somatization.
Study evaluation: After the assessment of the studies, the quality assessment (Risk of Bias Tool of the Cochrane Institute) and the extraction of the data were conducted. After that five meta-analyses were carried out for each of the five interventions using the Review Manager of the Cochrane Institute (RevMan 5.3)
Results: Acupuncture and dry needling were statistically significantly more effective in providing short-term pain relief compared to the control group in patients with low disability pain (p=0.04) and (p=0.02), respectively. Acupuncture or dry needling did not show a significant result in the improvement of MMO in the short-term period. Laser therapy is more effective in relieving pain (p<0.0001) and functional outcomes (p=0.03) in the short term compared to placebo for low disability pain. Botulinum toxin (p=0.003) and NSAIDs (p=0.03) showed significantly better short-term improvement in pain intensity for high disability pain. Low disability pain is significantly better treated by psychosocial interventions than by other treatments in terms of long-term pain relief (more than 12 months) (p=0.02). Patients with high disability pain had significantly lower depression scores after psychosocial interventions than after other treatments (p=0.008). Physiotherapy showed a statistically significant short-term analgesic effect in patients with high disability pain compared to placebo (p=0.04). Manual Therapy (MT) showed a statistically significant short-term analgesic effect in high disability pain compared to the control group (p=0.01). Patients with low disability pain showed a statistically significant short-term pain-relieving effect with the single intervention of MT in combination with exercise compared to the control groups (p=0.003). A statistically significant result in the improvement of MMO was found in the short-term period in low disability pain for the single interventions of physiotherapy (p=0.008) and physiotherapy in combination with another treatment compared to other treatments (p=0.03), MT compared to the control group (p=0.03) and physiotherapy compared to splint therapy (p=0.03). Clinical conclusion: Individual interventions of the five extraoral therapies confirm the hypothesis that painful TMDs respond differently to established therapies depending on the degree of chronic pain-related disability and that the prognosis of therapy is significantly influenced by the degree of chronic pain- related disability of the condition, according to the GCPS.

Die ängstliche Depression stellt einen Subtypus der Depression dar, der noch nicht ausreichend erforscht ist und somit eine Herausforderung im klinischen Alltag darstellt. Laut der bisherigen Literatur sind genetische Unterschiede sowie Kindheitstraumatisierungen an der Pathophysiologie von Depressionen beteiligt und mitverantwortlich für die Ausprägung des Subtypus ängstliche Depression.
In dieser Untersuchung wurde erforscht, ob es unterschiedliche Genexpressionslevel des Gens NR3C1 zwischen ängstlich-depressiven und nicht-ängstlich-depressiven Personen gibt. Zusätzlich wurde geprüft, ob Kindheitstraumatisierungen einen weiteren Einfluss auf die Genexpression der beiden Subtypen der Depression haben.
Es zeigte sich, dass ängstlich-depressive Personen in Woche 1 bis 4 höhere HAM-D-Summenwerte erzielten, mit zusätzlichen Kindheitstraumatisierungen wurden die höchsten HAM-D-Werte festgestellt. Diese Gruppe hatte gehäuft Kindheitstraumata im Fragebogen angegeben, die Traumata Emotionale Misshandlung und Körperliche Vernachlässigung kamen signifikant häufiger vor.
Anhand dieser durchgeführten Studie konnten zusammengefasst werden, dass sich die Genexpressionslevel von NR3C1 zwischen den beiden Subtypen als unterschiedlich erwies. Zusätzlich scheinen die beiden Kindheitstraumata Emotionale Misshandlung und Körperliche Vernachlässigung einen weiteren Einfluss auf die Genexpression von NR3C1 zu haben.
Die unterschiedliche Genexpression von NR3C1 deutet auf verschiedene Funktionsweisen des GR zwischen den Subtypen hin. Dies könnte für die Verlaufsbeurteilung und Therapieansätze der Erkrankung von Bedeutung sein. Die häufiger vorkommenden Kindheitstraumatisierungen bei ängstlich-depressiven Personen können als ein pathophysiologischer Baustein für die Entstehung der ängstlichen Depression gesehen werden. Daher ist es umso wichtiger, das Überprüfen von erlebten Kindheitstraumata bei initialer Befragung in den klinischen Alltag mitaufzunehmen. Da auch der Depressionsschweregrad durch Kindheitstraumatisierungen in dieser Studie zunahm, ergeben sich daraus mögliche Konsequenzen für die therapeutische Planung.
This work aims at elucidating chemical processes involving homogeneous catalysis and photo–physical relaxation of excited molecules in the solid state. Furthermore, compounds with supposedly small singlet–triplet gaps and therefore biradicaloid character are investigated with respect to their electro–chemical behavior. The work on hydroboration catalysis via a reduced 9,10–diboraanthracene (DBA) was preformed in collaboration with the Wagner group in Frankfurt, more specifically Dr. Sven Prey, who performed all laboratory experiments. The investigation of delayed luminescence properties in arylboronic esters in their solid state was conducted in collaboration with the Marder group in Würzburg. The author of this work took part in the synthesis of the investigated compounds while being supervised by Dr. Zhu Wu. The final project was a collaboration with the group of Anukul Jana from Hyderabad, India who provided the experimental data.
Cardiovascular disease is one of the leading causes of death worldwide and, so far, echocardiography, nuclear cardiology, and catheterization are the gold standard techniques used for its detection. Cardiac magnetic resonance (CMR) can replace the invasive imaging modalities and provide a "one-stop shop" characterization of the cardiovascular system by measuring myocardial tissue structure, function and perfusion of the heart, as well as anatomy of and flow in the coronary arteries. In contrast to standard clinical magnetic resonance imaging (MRI) scanners, which are often operated at a field strength of 1.5 or 3 Tesla (T), a higher resolution and subsequent cardiac parameter quantification could potentially be achieved at ultra-high field, i.e., 7 T and above.
Unique insights into the pathophysiology of the heart are expected from ultra-high field MRI, which offers enhanced image quality in combination with novel contrast mechanisms, but suffers from spatio-temporal B0 magnetic field variations. Due to the resulting spatial misregistration and intra-voxel dephasing, these B0-field inhomogeneities generate a variety of undesired image artifacts, e.g., artificial image deformation. The resulting macroscopic field gradients lead to signal loss, because the effective transverse relaxation time T2* is shortened. This affects the accuracy of T2* measurements, which are essential for myocardial tissue characterization. When steady state free precession-based pulse sequences are employed for image acquisition, certain off-resonance frequencies cause signal voids. These banding artifacts complicate the proper marking of the myocardium and, subsequently, systematic errors in cardiac function measurements are inevitable. Clinical MR scanners are equipped with basic shim systems to correct for occurring B0-field inhomogeneities and resulting image artifacts, however, these are not sufficient for the advanced measurement techniques employed for ultra-high field MRI of the heart.
Therefore, this work focused on the development of advanced B0 shimming strategies for CMR imaging applications to correct the spatio-temporal B0 field variations present in the human heart at 7 T. A novel cardiac phase-specific shimming (CPSS) technique was set up, which featured a triggered B0 map acquisition, anatomy-matched selection of the shim-region-of-interest (SROI), and calibration-based B0 field modeling. The influence of technical limitations on the overall spherical harmonics (SH) shim was analyzed. Moreover, benefits as well as pitfalls of dynamic shimming were debated in this study. An advanced B0 shimming strategy was set up and applied in vivo, which was the first implementation of a heart-specific shimming approach in human UHF MRI at the time.
The spatial B0-field patterns which were measured in the heart throughout this study contained localized spots of strong inhomogeneities. They fluctuated over the cardiac cycle in both size and strength, and were ideally addressed using anatomy-matched SROIs. Creating a correcting magnetic field with one shim coil, however, generated eddy currents in the surrounding conducting structures and a resulting additional, unintended magnetic field. Taking these shim-to-shim interactions into account via calibration, it was demonstrated for the first time that the non-standard 3rd-order SH terms enhanced B0-field homogeneity in the human heart. However, they were attended by challenges for the shim system hardware employed in the presented work, which was indicated by the currents required to generate the optimal 3rd-order SH terms exceeding the dynamic range of the corresponding shim coils. To facilitate dynamic shimming updated over the cardiac cycle for cine imaging, the benefit of adjusting the oscillating CPSS currents was found to be vital. The first in vivo application of the novel advanced B0 shimming strategy mostly matched the simulations.
The presented technical developments are a basic requirement to quantitative and functional CMR imaging of the human heart at 7 T. They pave the way for numerous clinical studies about cardiac diseases, and continuative research on dedicated cardiac B0 shimming, e.g., adapted passive shimming and multi-coil technologies.
The transcription factor SPT5 physically interacts with MYC oncoproteins and is essential for efficient transcriptional activation of MYC targets in cultured cells. Here, we use Drosophila to address the relevance of this interaction in a living organism. Spt5 displays moderate synergy with Myc in fast proliferating young imaginal disc cells. During later development, Spt5-knockdown has no detectable consequences on its own, but strongly enhances eye defects caused by Myc overexpression. Similarly, Spt5-knockdown in larval type 2 neuroblasts has only mild effects on brain development and survival of control flies, but dramatically shrinks the volumes of experimentally induced neuroblast tumors and significantly extends the lifespan of tumor-bearing animals. This beneficial effect is still observed when Spt5 is knocked down systemically and after tumor initiation, highlighting SPT5 as a potential drug target in human oncology.
Die C1q/tumor necrosis factor-related proteins (CTRPs) sind eine Ligandenfamilie aus sezernierten Plasmaproteinen, welche sich in ihrem Grundbauplan ähneln.
Daten aus der Literatur deuten darauf hin, dass sie zum Teil positive Effekte auf den Stoffwechsel und das Herz-Kreislaufsystem besitzen und somit eine mögliche therapeutische Zielstruktur darstellen. Während für manche CTRPs bereits Rezeptoren identifiziert werden konnten, ist für andere immer noch nicht geklärt, an welche Rezeptoren sie binden oder über welche sie diese Wirkungen erzielen. Um die CTRPs zukünftig therapeutisch nutzen zu können, muss die Wirkung der CTRPs auf verschiedene Zellen weiter analysiert werden. Dafür wurden in dieser Arbeit Zellen, auf die Expression bereits bekannter CTRP-Rezeptoren hin, untersucht. Des Weiteren wurden die durch CTRP2, CTRP3, CTRP4, CTRP9A, CTRP10, CTRP11, CTRP13 und CTRP14 induzierten Änderungen in der ATP- und Laktatproduktion als Surrogatparameter für Kardiotoxizität in den Kardiomyozytenzelllinien H9c2 und AC16 getestet, um potenziell kardiotoxische Wirkungen frühzeitig erkennen zu können. Es konnte gezeigt werden, dass die CTRPs sicher für Kardiomyozyten zu sein scheinen, was eine wichtige Grundlage für die therapeutische Nutzbarkeit darstellt.
Ziel dieser klinisch-experimentellen Studie war die Untersuchung
elektromyographischer Kaumuskelprofile von beschwerdefreien Probandinnen
unterschiedlichen Bruxismusgrades nach sensomotorischem Training. Die aufgestellte
Hypothese postulierte signifikante Unterschiede der EMG-Parameter nach
sensomotorischem Training. Nach einer Ruhephase ohne Intervention sollten die
Unterschiede in den Ausgangszustand zurückkehren. Hierzu wurden 40 Probandinnen
mit einem Durchschnittsalter von 24,58 ± 2,72 Jahren über einen Zeitraum von fünf
Wochen untersucht. Die Probandinnen wurden mittels zufälliger Verteilung und
altersentsprechend gematcht in zwei gleichgroße Gruppen eingeteilt. Sowohl die
Teilnehmerinnen der Kontrollgruppe, als auch die der Interventionsgruppe absolvierten
im Verlauf der Studie drei elektromyographische Messungen. Nach einer einwöchigen
Voruntersuchungsphase fand die erste Messung (T1) statt. Nach drei Wochen und nach
fünf Wochen erfolgten die zweite (T2) und die dritte Messung (T3). Während der
Messungen führten die Probandinnen kraftkontrollierte Übungen mit drei
submaximalen Kraftleveln und maximalen Kräften aus. Zusätzlich absolvierte die
Interventionsgruppe zwischen T1 und T2 ein sensomotorisches Training mit dem
RehaBite®-Gerät. Die bipolaren Oberflächen-EMG-Ableitungen erfolgten für beide Mm.
masseteres und Mm. temporales. Insgesamt wurden acht Muskelareale aufgezeichnet.
Sechs für die Mm. masseteres und zwei für die Mm. temporales. Die submaximalen
Kräfte wurden als RMS %MVC und die maximalen Kräfte als RMS MVC verglichen. Die
statistischen Vergleiche erfolgten anhand von T-Tests und Mixed ANOVAs. Nach
Beurteilung der Ergebnisse konnte kein signifikanter Effekt des sensomotorischen
Trainings identifiziert werden. Die aufgestellte Hypothese muss daher abgelehnt
werden. Für das erste der drei submaximalen Kraftlevel konnte für die Initialmessung
(T1) ein signifikanter Unterschied zwischen Probandinnen mit und ohne Schlafbruxismus
in zwei der acht Muskelareale festgestellt werden. Für zukünftige Folgeuntersuchungen
zur Wirksamkeit des sensomotorischen Trainings bei Bruxismus ist die Verlängerung des
Interventionsintervalls sowie eine Vergrößerung des Studienkollektivs samt Einschluss
männlicher Probanden empfehlenswert.
Avulsionsfrakturen des Kalkaneus sind seltene Verletzungen und machen mit 0,03 % bis 0,1 % einen sehr kleinen Anteil aller Frakturen aus. (13, 20-23, 25) Allerdings sind sie mit einer hohen Rate an Komplikationen verbunden. (27, 30, 73) Neben der prekären Weichteilsituation (17, 24, 30, 43, 44, 49) stellt vor allem eine ausreichende Stabilität der osteosynthetischen Versorgung eine Herausforderung dar. (30, 73) In dieser biomechanischen Studie wurden drei verschiedene kanülierte Schraubentypen, sowie zwei winkelstabile Plattenosteosynthesen zur Versorgung von Kalkaneusfrakturen bezüglich ihrer biomechanischen Stabilität in einer Materialprüfmaschine unter optischem Tracking mithilfe einer 3D-Kamera getestet und verglichen. Dazu wurden für jede der fünf Gruppen Avulsionsfrakturen vom Typ II nach Beavis an je zehn Kalkaneusmodellen aus Kunststoff erzeugt und diese anschließend unter Verwendung der jeweiligen Osteosynthese versorgt. Unter den drei Schraubentypen gab es zwei kanülierte Schrauben unterschiedlicher Größe mit Unterlegscheiben, die auch in der klinischen Praxis bereits Verwendung finden. Außerdem wurden versenkbare, kanülierte Doppelgewinde Schrauben verwendet, deren Einsatz bei dieser Verletzung nach unserem Wissen bisher nicht in der Literatur beschrieben ist. Das winkelstabile Plattensystem wurde bis jetzt nach eigener Literaturrecherche ebenfalls nicht in der hier angewandten Art und Weise zur Versorgung derartiger Frakturen verwendet. Alle Versuchsmodelle wurden, sofern es nicht während der Testung zum Versagen kam, auf drei verschiedenen Kraftniveaus (100 N, 200 N, 300 N) zyklisch und anschließend mit einer Maximalkrafttestung getestet. Dabei wurden das Peak to Peak Displacement bei 100 N, 200 N und 300 N, das maximale Displacement, die plastische Deformation bei 100 N, 200 N und 300 N, die Maximalkraft, die Steifigkeit bei 100 N, 200 N und 300 N und die Art des Fixationsversagens erfasst. Ziel der Studie war es, Unterschiede zwischen den Versorgungsformen aufzudecken. Die Ergebnisse zeigen, dass die winkelstabile Plattenosteosynthese Stabilitätsdefizite bei der Versorgung von „beak“ Frakturen aufweist. Außerdem konnte gezeigt werden, dass beim Vergleich der versenkbaren, kanülierten Doppelgewinde Schrauben mit 5,0 mm Durchmesser mit den kanülierten Schrauben mit 6,5 mm Durchmesser und Unterlegscheiben keine statistisch signifikanten Unterschiede bestehen, außer bezüglich der Steifigkeit bei 300 N. Somit ist eine vergleichbare biomechanische Stabilität wahrscheinlich. Die versenkbaren, kanülierten Doppelgewinde Schrauben besitzen eine hohe biomechanische Stabilität und bieten die Möglichkeit, den Schraubenkopf im Knochenniveau zu versenken, wodurch weniger Weichteilirritationen und Wundheilungsstörungen zu vermuten sind. Sie scheinen deshalb eine attraktive Alternative zu kanülierten Schrauben mit Unterlegscheiben zu sein. Ob versenkbare, kanülierte Doppelgewinde Schrauben z.B. auch im Kadaverversuch eine ausreichende biomechanische Stabilität zeigen, bleibt allerdings nachfolgenden biomechanischen Studien vorbehalten. Ebenso muss der mögliche postoperative Vorteil hinsichtlich der Schonung der Weichteile in klinischen Studien untersucht werden.
Der kathetergestützte Aortenklappenersatz nimmt auch bei Patienten mit niedrigem OP-Risiko einen zunehmend größeren Stellenwert zur Behandlung der hochgradigen Aortenklappenstenose ein.45 Umso wichtiger ist es, die einzelnen Schritte der Intervention zu optimieren. In einigen Arbeiten wurde bereits die Vordilatation als obsolet bezeichnet, da sie lediglich die OP-Zeit verlängere und Komplikationen wie Schlaganfälle und AV-Blockierungen begünstige.22,52,53,57,59 Ziel dieser Studie war es, die Vor- und Nachteile der Vordilatation zu untersuchen. Hierzu wurden 625 Patienten, die im Zeitraum von 2016-2020 eine TAVI am UKW erhielten, retrospektiv analysiert (323 mit, 302 ohne Vordilatation). Es wurden demographische sowie prä-, peri- und post-interventionelle Daten analysiert. Statistisch signifikante Unterschiede wurden bei den Schlaganfällen beobachtet (p=0,01), die mit 2,2% lediglich bei Patienten mit Vordilatation auftraten, sodass bei einem hohen Schlaganfallrisiko hierauf verzichtet werden sollte. Zusätzlich war in der Gruppe mit Vordilatation die passagere Schrittmacherabhängigkeit signifikant häufiger (p=0,01). Alle anderen Komplikationen waren nicht signifikant. In beiden Gruppen zeigte sich zu >95% ein Device-Success, sodass der Verzicht auf eine Prädilatation nicht mit einem schlechteren Outcome assoziiert und somit sicher ist.53,57,58,59,61
Die Auswertung der TTE-Daten zeigte, dass eine Prädilatation durchgeführt wurde, wenn die Klappe signifikant höhergradig stenosiert war (Pmean 50,17 vs. 46,79mmHG). Ferner wurde bei leichtgradigen Aortenklappeninsuffizienzen signifikant häufiger auf eine Vordilatation verzichtet (p=0,04). Eine Vordilatation kann also bei komplexeren anatomischen Verhältnissen sinnvoll sein, um einen optimalen Klappensitz zu gewährleisten.52,53 Nach TAVI zeigte sich die LV-EF in der Gruppe mit Prädilatation signifikant höher (p=0,002). Höhergradige Aortenklappeninsuffizienzen scheinen nicht durch eine Vordilatation begünstigt zu sein, die AI°II wurde nur bei 4 Patienten ohne Vordilatation beobachtet. In den postinterventionellen EKG-Daten zeigten sich in der Gruppe ohne Vordilatation signifikant häufiger Linksschenkelblöcke sowie ein AVB °II, Typ II, was vermutlich durch die fehlende Vorbereitung der Klappe und den damit assoziierten ungünstigeren Prothesensitz zu erklären ist.53 Die Nachdilatation wurde nicht durch eine vorausgegangene Vordilatation beeinflusst. Bezüglich der implantierten Klappenarten wurde die S3 Ultra signifikant häufiger bei Patienten ohne Vordilatation eingesetzt. Die in vielen Arbeiten beschriebene kürzere OP-Dauer ließ sich in dieser Studie nicht bestätigen.52,53,56 Stattdessen war bei TAVIs ohne Vordilatation die Eingriffsdauer im Schnitt 4min länger (p=0,11). Es bestätigte sich, dass bei einer Prädilatation signifikant mehr Kontrastmittel verwendet wurde (p=0,001) und die Strahlenbelastung höher war. Dies ist insbesondere für Patienten mit einer Niereninsuffizienz von Bedeutung.42 Ob eine Vordilatation durchgeführt wird, sollte also individuell aufgrund der Begleiterkrankungen und Risikofaktoren entschieden werden.
Anxiety patients overgeneralize fear, also because of an inability to perceptually discriminate threat and safety signals. Therefore, some studies have developed discrimination training that successfully reduced the occurrence of fear generalization. The present work is the first to take a treatment-like approach by using discrimination training after generalization has occurred. Therefore, two studies were conducted with healthy participants using the same fear conditioning and generalization paradigm, with two faces as conditioned stimuli (CSs), and four facial morphs between CSs as generalization stimuli (GSs). Only one face (CS+) was followed by a loud scream (unconditioned stimulus, US). In Study 1, participants underwent either fear-relevant (discriminating faces) or fear-irrelevant discrimination training (discriminating width of lines) or a non-discriminative control training between the two generalization tests, each with or without feedback (n = 20 each). Generalization of US expectancy was reduced more effectively by fear-relevant compared to fear-irrelevant discrimination training. However, neither discrimination training was more effective than non-discriminative control training. Moreover, feedback reduced generalization of US expectancy only in discrimination training. Study 2 was designed to replicate the effects of the discrimination-training conditions in a large sample (N = 244) and examine their benefits in individuals at risk for anxiety disorders. Again, feedback reduced fear generalization particularly well for US expectancy. Fear relevance was not confirmed to be particularly fear-reducing in healthy participants, but may enhance training effects in individuals at risk of anxiety disorder. In summary, this work provides evidence that existing fear generalization can be reduced by discrimination training, likely involving several (higher-level) processes besides perceptual discrimination (e.g., motivational mechanisms in feedback conditions). Its use may be promising as part of individualized therapy for patients with difficulty discriminating similar stimuli.
Die Dissertation befasst sich mit der Reaktivität von 1,2-Bis(dichlorboryl)benzol. Im ersten Kapitel wird auf die Problematik bei dessen Synthese eingegangen. Der zweite Teil der Arbeit befasst sich mit der Bildung von entsprechenden Boran-Addukten mit verschiedenen Lewis-Basen. Das dritte Kapitel beschreibt die Synthese eines neuartigen, vollständig ungesättigten 1,2-Diboretdiradikals, welches durch die schrittweise Reduktion des 1,2-[(CAAC)BCl2]2-Benzols erhalten wurde. Darüber hinaus konnte bei dieser schrittweisen Reduktion ebenfalls das einfache Borylradikal, das nicht-cyclische Diradikal und das dianionische gespannte C2B2-Ringsystem erhalten werden. Anfängliche Reaktivitätsstudien zum 1,2-Diboretdiradikal zeigen zudem, dass die B-B-Bindung durch Umsetzung mit Kohlenstoffmonoxid gespalten und so ein Bisborylen dargestellt werden kann. Im vierten Kapitel konnte das 1,2-Bis(dichlorboryl)benzol durch Transmetallierungsreaktionen zu verschiedenen, sich in ihren Eigenschaften stark unterscheidenden, Verbindungen umgesetzt werden. So konnte das fluoreszierende ortho-phenylenverbrückte Bis-9-Borafluoren erhalten werden, aus welchem durch Wärmezufuhr das ebenfalls fluoreszierendes diboraanthracenartige Umlagerungsprodukt gewonnen werden konnte. Beide Verbindungen wurden auf ihre photophysikalischen und elektrochemischen Eigenschaften untersucht. Weiterhin konnten polycyclische Boracyclen mit C10B2-Gerüst erhalten werden, bei welchen instantan die selektive Bildung von zwei chiralen Zentren über eine Vielzahl an B-C-Bindungsbrüchen und -knüpfungen beobachtet wurde. Zuletzt konnte ein thermisch empfindliches, potentiell explosives Azid-verbrücktes Azidoboran dargestellt werden, bei welchem eine Staudinger-artige Reaktivität beobachtet werden konnte.
Hereditary spastic paraplegias (HSPs) are genetically-determined, neurodegenerative disorders characterized by progressive weakness and spasticity of the lower limbs. Spastic paraplegia type 11 (SPG11) is a complicated form of HSP, which is caused by mutations in the SPG11 gene encoding spatacsin, a protein possibly involved in lysosomal reformation. Based on our previous studies demonstrating that secondary neuroinflammation can be a robust amplifier of various genetically-mediated diseases of both the central and peripheral nervous system, we here test the possibility that neuroinflammation may modify the disease outcome also in a mouse model for SPG11. Spg11-knockout (Spg11-/-) mice develop early walking pattern and behavioral abnormalities, at least partially reflecting motor, and behavioral changes typical for patients. Furthermore, we detected a progressive increase in axonal damage and axonal spheroid formation in the white and grey matter compartments of the central nervous system of Spg11-/- mice. This was accompanied by a concomitant substantial increase of secondary inflammation by cytotoxic CD8+ and CD4+ T-lymphocytes. We here provide evidence that disease-related changes can be ameliorated/delayed by the genetic deletion of the adaptive immune system. Accordingly, we provide evidence that repurposing clinically approved immunomodulators (fingolimod/FTY720 or teriflunomide), that are in use for treatment of multiple sclerosis (MS), also improve disease symptoms in mice, when administered in an early (before neural damage) or late (after/during neural damage) treatment regime.
This work provides strong evidence that immunomodulation can be a therapeutic option for the still untreatable SPG11, including its typical neuropsychological features. This poses the question if inflammation is not only a disease amplifier in SPG11 but can act as a unifying factor also for other genetically mediated disorders of the CNS. If true, this may pave the way to therapeutic options in a wide range of still untreatable, primarily genetic, neurological disorders by repurposing approved immunomodulators.

Das Phänomen der Eskalation von Auseinandersetzungen zwischen Konfliktparteien ist seitens der Philosophie und Ethik bislang kaum untersucht worden. Dietmar Hübner nimmt sich dieses Desiderates an und analysiert die normative Tiefenstruktur von Eskalationen als Schlüssel zu einem Verständnis der Stabilität eskalierender Konflikte, ihrer psychosozialen Dynamik sowie paradoxal erscheinender Ausdrucksformen.
Biological systems are in dynamic interaction. Many responses reside in the core concepts of biological systems interplay (competition and cooperation). In infection situation, the competition between a bacterial system and a host is shaped by many stressors at spatial and temporal determinants. Reactive chemical species are universal stressors against all biological systems since they potentially damage the basic requirements of these systems (nucleic acids, proteins, carbohydrates, and lipids). Either produced endogenously or exogenously, reactive chemical species affect the survival of pathogens including the gram-positive
Staphylococcus aureus (S. aureus). Therefore, bacteria developed strategies to overcome the toxicity of reactive species.
S. aureus is a widely found opportunistic pathogen. In its niche, S. aureus is in permanent contact with surrounding microbes and host factors. Deciphering the deterministic factors
in these interactions could facilitate pinpointing novel bacterial targets. Identifying
the aforementioned targets is crucial to develop new strategies not only to kill the pathogenic organisms but also to enhance the normal flora to minimize the pathogenicity and virulence of potential pathogens. Moreover, targeting S. aureus stress response can be used
to overcome bacterial resistance against host-derived factors. In this study, I identify a novel
S. aureus stress response factor against reactive electrophilic, oxygen, and hypochlorite species to better understand its resilience as a pathogen.
Although bacterial stress response is an active research field, gene function is a current bottleneck in characterizing the understudied bacterial strategies to mediate stress conditions. I aimed at understanding the function of a novel protein family integrated
in many defense systems of several biological systems.
In bacteria, fungi, and plants, old yellow enzymes (OYEs) are widely found. Since the first isolation of the yellow flavoprotein, OYEs are used as biocatalysts for decades to reduce activated C=C bonds in α,β-unsaturated carbonyl compounds. The promiscuity
of the enzymatic catalysis is advantageous for industrial applications.
However, the physiological function of OYEs, especially in bacteria, is still puzzling.
Moreover, the relevance of the OYEs in infection conditions remained enigmatic.
Here, I show that there are two groups of OYEs (OYE flavin oxidoreductase, OfrA and OfrB) that are encoded in staphylococci and some firmicutes. OfrA (SAUSA300_0859) is more conserved than OfrB (SAUSA300_0322) in staphylococci and is a part of the staphylococcal core genome.
A reporter system was established to report for ofrA in S. aureus background.
The results showed that ofrA is induced under electrophilic, oxidative, and hypochlorite stress. OfrA protects S. aureus against quinone, methylglyoxal, hydrogen peroxide,
and hypochlorite stress. Additionally, the results provide evidence that OfrA supports
thiol-dependent redox homeostasis. At the host-pathogen interface, OfrA promotes S. aureus fitness in murine macrophage cell line. In whole human blood, OfrA is involved in S. aureus survival indicating a potential clinical relevance to bacteraemia.
In addition, ofrA mutation affects the production of the virulence factor staphyloxanthin via the upper mevalonate pathway. In summary, decoding OfrA function and its proposed mechanism of action in S. aureus shed the light on a conserved stress response within multiple organisms.
In the scope of climate warming and the increase in frequency and intensity of severe heat waves in Central Europe, identification of temperate tree species that are suited to cope with these environmental changes is gaining increasing importance. A number of tree physiological characteristics are associated with drought-stress resistance and survival following severe heat, but recent studies have shown the importance of plant hydraulic and anatomical traits for predicting drought-induced tree mortality, such as vessel diameter, and their potential to predict species distribution in a changing climate.
A compilation of large global datasets is required to determine traits related to drought-induced embolism and test whether embolism resistance can be determined solely by anatomical traits. However, most measurements of plant hydraulic traits are labour-intense and prone to measurement artefacts. A fast, accurate and widely applicable technique is necessary for estimating xylem embolism resistance (e.g., water potential at 50% loss of conductivity, P50), in order to improve forecasts of future forest changes. These traits and their combination must have evolved following the selective pressure of the environmental conditions in which each species occurs. Describing these environmental-trait relationships can be useful to assess potential responses to environmental change and mitigation strategies for tree species, as future warmer temperatures may be compounded by drier conditions.
Theory and simulation of ultrafast autodetachment dynamics and nonradiative relaxation in molecules

In this thesis, theoretical approaches for the simulation of electron detachment processes in molecules following vibrational or electronic excitation are developed and applied. These approaches are based on the quantum-classical surface-hopping methodology, in which nuclear motion is treated classically as an ensemble of trajectories in the potential of quantum-mechanically described electronic degrees of freedom.
Das zeitgleiche Auftreten eines ischämischen Schlaganfalls sowie eines Takotsubo-Syndroms (TTS) scheint eine relevante, bisher nicht ausreichend verstandene klinische Konstellation zu sein. Die Pathologien können als über die Hirn-Herz-Achse gekoppelt verstanden werden, in die die Blut-Hirn-Schranke (BHS) als funktionale Komponente integriert ist. Das klinisch-neurologische Outcome dieses Patient:innen-Kollektivs scheint signifikant schlechter zu sein als nach solitärem ischämischen Insult. Es wurde hypothetisiert, dass die BHS in besonderem Maße kompromittiert sein könnte. Das vorwiegend weibliche, postmenopausale Patient:innenkollektiv präsentierte laborchemisch elevierte Katecholaminspiegel sowie Entzündungsparameter. Diese Konditionen wurden unter Sauerstoff-Glucose-Entzug (OGD) in vitro simuliert und resultierende Alterationen eines etablierten BHS-Modells aus murinen cEND-Zellen der cerebralen Mikrozirkulation untersucht. Die Evaluation der BHS-Integrität erfolgte anhand von spezifischen Junktionsproteinen sowie Integrinuntereinheiten. Alle Versuche wurden parallel unter Östrogen-Applikation (E2) durchgeführt, um die mögliche BHS-Protektion durch das weibliche Sexualhormon zu untersuchen. Die getrennte Applikation von Katecholaminen (KAT) sowie Entzündungsmediatoren (INF) führte gegenüber der simultanen Applikation zu einem geringeren BHS-Schaden. Dieser erschien zeitgebunden, wobei sich das Ausmaß gewissermaßen proportional zur Einwirkdauer verhielt. Auswirkungen von OGD sowie einer Reoxygenierung, im Sinne einer simulierten Reperfusion, potenzierten sich mit den Effekten von KAT/INF. Überwiegend kompromittierten OGD und KAT/INF die BHS-Integrität, wobei nach Reoxygenierung eine „Erholung“ oder ein „Reperfusionsschaden“ vorlag. Eine Protektion durch E2 war morphologisch nachweisbar, speziell gegenüber OGD, KAT/INF sowie einem „Reperfusionsschaden“. Auf Ebene der Gen- sowie Proteinexpression konnte dies nicht gezeigt werden. Die Homöostase des ZNS würde in vivo beeinträchtigt, Katecholamine sowie Entzündungsmediatoren könnten ungehindert das bereits durch die Ischämie geschädigte neuronale Gewebe erreichen. Insgesamt trägt diese Arbeit zu einem Verständnis der molekularen BHS-Veränderungen im Kontext des zeitgleichen Auftretens von TTS und einem ischämischem Insult bei. Es wurde eine experimentelle Grundlage geschaffen, um zukünftig pathogenetische Hintergründe weiter erforschen zu können. Darauf aufbauend könnten, nach weiterer in vitro- sowie in vivo-Forschung, klinische Therapiekonzepte optimiert werden.
The emergence of human induced pluripotent stem cells (iPSCs) and the rise of the clustered regularly interspaced short palindromic repeats/CRISPR-associated protein 9 (CRISPR/Cas9) gene editing technology innovated the research platform for scientists based on living human pluripotent cells. The revolutionary combination of both Nobel Prize-honored techniques enables direct disease modeling especially for research focused on genetic diseases. To allow the study on mutation-associated pathomechanisms, we established robust human in vitro systems of three inherited cardiomyopathies: arrhythmogenic cardiomyopathy (ACM), dilated cardiomyopathy with juvenile cataract (DCMJC) and dilated cardiomyopathy with ataxia (DCMA).
Sendai virus vectors encoding OCT3/4, SOX2, KLF4, and c-MYC were used to reprogram human healthy control or mutation-bearing dermal fibroblasts from patients to an embryonic state thereby allowing the robust and efficient generation of in total five transgene-free iPSC lines. The nucleofection-mediated CRISPR/Cas9 plasmid delivery in healthy control iPSCs enabled precise and efficient genome editing by mutating the respective disease genes to create isogenic mutant control iPSCs. Here, a PKP2 knock-out and a DSG2 knock-out iPSC line were established to serve as a model of ACM. Moreover, a DNAJC19 C-terminal truncated variant (DNAJC19tv) was established to mimic a splice acceptor site mutation in DNAJC19 of two patients with the potential of recapitulating DCMA-associated phenotypes. In total eight self-generated iPSC lines were assessed matching internationally defined quality control criteria. The cells retained their ability to differentiate into cells of all three germ layers in vitro and maintained a stable karyotype. All iPSC lines exhibited a typical stem cell-like morphology as well as expression of characteristic pluripotency markers with high population purities, thus validating the further usage of all iPSC lines in in vitro systems of ACM, DCMA and DCMJC.
Furthermore, cardiac-specific disease mechanisms underlying DCMA were investigated using in vitro generated iPSC-derived cardiomyocytes (iPSC-CMs). DCMA is an autosomal recessive disorder characterized by life threatening early onset cardiomyopathy associated with a metabolic syndrome. Causal mutations were identified in the DNAJC19 gene encoding an inner mitochondrial membrane (IMM) protein with a presumed function in mitochondrial biogenesis and cardiolipin (CL) remodeling. In total, two DCMA patient-derived iPSC lines (DCMAP1, DCMAP2) of siblings with discordant cardiac phenotypes, a third isogenic mutant control iPSC line (DNAJC19tv) as well as two control lines (NC6M and NC47F) were directed towards the cardiovascular lineage upon response to extracellular specification cues. The monolayer cardiac differentiation approach was successfully adapted for all five iPSC lines and optimized towards ventricular subtype identity, higher population purities and enhanced maturity states to fulfill all DCMA-specific requirements prior to phenotypic investigations. To provide a solid basis for the study of DCMA, the combination of lactate-based metabolic enrichment, magnetic-activated cell sorting, mattress-based cultivation and prolonged cultivation time was performed in an approach-dependent manner. The application of the designated strategies was sufficient to ensure adult-like characteristics, which included at least 60-day-old iPSC-CMs. Therefore, the novel human DCMA platform was established to enable the study of the pathogenesis underlying DCMA with respect to structural, morphological and functional changes.
The disease-associated protein, DNAJC19, is constituent of the TIM23 import machinery and can directly interact with PHB2, a component of the membrane bound hetero-oligomeric prohibitin ring complexes that are crucial for phospholipid and protein clustering in the IMM. DNAJC19 mutations were predicted to cause a loss of the DnaJ interaction domain, which was confirmed by loss of full-length DNAJC19 protein in all mutant cell lines. The subcellular investigation of DNAJC19 demonstrated a nuclear restriction in mutant iPSC-CMs. The loss of DNAJC19 co-localization with mitochondrial structures was accompanied by enhanced fragmentation, an overall reduction of mitochondrial mass and smaller cardiomyocytes. Ultrastructural analysis yielded decreased mitochondria sizes and abnormal cristae providing a link to defects in mitochondrial biogenesis and CL remodeling. Preliminary data on CL profiles revealed longer acyl chains and a more unsaturated acyl chain composition highlighting abnormities in the phospholipid maturation in DCMA.
However, the assessment of mitochondrial function in iPSCs and dermal fibroblasts revealed an overall higher oxygen consumption that was even more enhanced in iPSC-CMs when comparing all three mutants to healthy controls. Excess oxygen consumption rates indicated a higher electron transport chain (ETC) activity to meet cellular ATP demands that probably result from proton leakage or the decoupling of the ETC complexes provoked by abnormal CL embedding in the IMM.
Moreover, in particular iPSC-CMs presented increased extracellular acidification rates that indicated a shift towards the utilization of other substrates than fatty acids, such as glucose, pyruvate or glutamine. The examination of metabolic features via double radioactive tracer uptakes (18F-FDG, 125I-BMIPP) displayed significantly decreased fatty acid uptake in all mutants that was accompanied by increased glucose uptake in one patient cell line only, underlining a highly dynamic preference of substrates between mutant iPSC-CMs.
To connect molecular changes directly to physiological processes, insights on calcium kinetics, contractility and arrhythmic potential were assessed and unraveled significantly increased beating frequencies, elevated diastolic calcium concentrations and a shared trend towards reduced cell shortenings in all mutant cell lines basally and upon isoproterenol stimulation. Extended speed of recovery was seen in all mutant iPSC-CMs but most striking in one patient-derived iPSC-CM model, that additionally showed significantly prolonged relaxation times. The investigations of calcium transient shapes pointed towards enhanced arrhythmic features in mutant cells comprised by both the occurrence of DADs/EADs and fibrillation-like events with discordant preferences.
Taken together, new insights into a novel in vitro model system of DCMA were gained to study a genetically determined cardiomyopathy in a patient-specific manner upon incorporation of an isogenic mutant control. Based on our results, we suggest that loss of full-length DNAJC19 impedes PHB2-complex stabilization within the IMM, thus hindering PHB-rings from building IMM-specific phospholipid clusters. These clusters are essential to enable normal CL remodeling during cristae morphogenesis. Disturbed cristae and mitochondrial fragmentation were observed and refer to an essential role of DNAJC19 in mitochondrial morphogenesis and biogenesis. Alterations in mitochondrial morphology are generally linked to reduced ATP yields and aberrant reactive oxygen species production thereby having fundamental downstream effects on the cardiomyocytes` functionality. DCMA-associated cellular dysfunctions were in particular manifested in excess oxygen consumption, altered substrate utilization and abnormal calcium kinetics. The summarized data highlight the usage of human iPSC-derived CMs as a powerful tool to recapitulate DCMA-associated phenotypes that offers an unique potential to identify therapeutic strategies in order to reverse the pathological process and to pave the way towards clinical applications for a personalized therapy of DCMA in the future.

Background
Dilated cardiomyopathy with ataxia (DCMA) is an autosomal recessive disorder arising from truncating mutations in DNAJC19, which encodes an inner mitochondrial membrane protein. Clinical features include an early onset, often life-threatening, cardiomyopathy associated with other metabolic features. Here, we aim to understand the metabolic and pathophysiological mechanisms of mutant DNAJC19 for the development of cardiomyopathy.
Methods
We generated induced pluripotent stem cell-derived cardiomyocytes (iPSC-CMs) of two affected siblings with DCMA and a gene-edited truncation variant (tv) of DNAJC19 which all lack the conserved DnaJ interaction domain. The mutant iPSC-CMs and their respective control cells were subjected to various analyses, including assessments of morphology, metabolic function, and physiological consequences such as Ca\(^{2+}\) kinetics, contractility, and arrhythmic potential. Validation of respiration analysis was done in a gene-edited HeLa cell line (DNAJC19tv\(_{HeLa}\)).
Results
Structural analyses revealed mitochondrial fragmentation and abnormal cristae formation associated with an overall reduced mitochondrial protein expression in mutant iPSC-CMs. Morphological alterations were associated with higher oxygen consumption rates (OCRs) in all three mutant iPSC-CMs, indicating higher electron transport chain activity to meet cellular ATP demands. Additionally, increased extracellular acidification rates suggested an increase in overall metabolic flux, while radioactive tracer uptake studies revealed decreased fatty acid uptake and utilization of glucose. Mutant iPSC-CMs also showed increased reactive oxygen species (ROS) and an elevated mitochondrial membrane potential. Increased mitochondrial respiration with pyruvate and malate as substrates was observed in mutant DNAJC19tv HeLa cells in addition to an upregulation of respiratory chain complexes, while cellular ATP-levels remain the same. Moreover, mitochondrial alterations were associated with increased beating frequencies, elevated diastolic Ca\(^{2+}\) concentrations, reduced sarcomere shortening and an increased beat-to-beat rate variability in mutant cell lines in response to β-adrenergic stimulation.
Conclusions
Loss of the DnaJ domain disturbs cardiac mitochondrial structure with abnormal cristae formation and leads to mitochondrial dysfunction, suggesting that DNAJC19 plays an essential role in mitochondrial morphogenesis and biogenesis. Moreover, increased mitochondrial respiration, altered substrate utilization, increased ROS production and abnormal Ca\(^{2+}\) kinetics provide insights into the pathogenesis of DCMA-related cardiomyopathy.
Electrochemical impedance spectroscopy (EIS) is a valuable technique analyzing electrochemical behavior of biological systems such as electrical characterization of cells and biomolecules, drug screening, and biomaterials in biomedical field. In EIS, an alternating current (AC) power signal is applied to the biological system, and the impedance of the system is measured over a range of frequencies.
In vitro culture models of endothelial or epithelial barrier tissue can be achieved by culturing barrier tissue on scaffolds made with synthetic or biological materials that provide separate compartments (apical and basal sides), allowing for further studies on drug transport. EIS is a great candidate for non-invasive and real-time monitoring of the electrical properties that correlate with barrier integrity during the tissue modeling. Although commercially available transendothelial/transepithelial electrical resistance (TEER) measurement devices are widely used, their use is particularly common in static transwell culture. EIS is considered more suitable than TEER measurement devices in bioreactor cultures that involve dynamic fluid flow to obtain accurate and reliable measurements. Furthermore, while TEER measurement devices can only assess resistance at a single frequency, EIS measurements can capture both resistance and capacitance properties of cells, providing additional information about the cellular barrier's characteristics across various frequencies. Incorporating EIS into a bioreactor system requires the careful optimization of electrode integration within the bioreactor setup and measurement parameters to ensure accurate EIS measurements. Since bioreactors vary in size and design depending on the purpose of the study, most studies have reported using an electrode system specifically designed for a particular bioreactor. The aim of this work was to produce multi-applicable electrodes and established methods for automated non-invasive and real-time monitoring using the EIS technique in bioreactor cultures. Key to the electrode material, titanium nitride (TiN) coating was fabricated on different substrates (materials and shape) using physical vapor deposition (PVD) and housed in a polydimethylsiloxane (PDMS) structure to allow the electrodes to function as independent units. Various electrode designs were evaluated for double-layer capacitance and morphology using EIS and scanning electron microscopy (SEM), respectively. The TiN-coated tube electrode was identified as the optimal choice. Furthermore, EIS measurements were performed to examine the impact of influential parameters related to culture conditions on the TiN-coated electrode system. In order to demonstrate the versatility of the electrodes, these electrodes were then integrated into in different types of perfusion bioreactors for monitoring barrier cells. Blood-brain barrier (BBB) cells were cultured in the newly developed dynamic flow bioreactor, while human umblical vascular endothelial cells (HUVECs) and Caco-2 cells were cultured in the miniature hollow fiber bioreactor (HFBR). As a result, the TiN-coated tube electrode system enabled investigation of BBB barrier integrity in long-term bioreactor culture. While EIS measurement could not detect HUVECs electrical properties in miniature HFBR culture, there was the possibility of measuring the barrier integrity of Caco-2 cells, indicating potential usefulness for evaluating their barrier function. Following the bioreactor cultures, the application of the TiN-coated tube electrode was expanded to hemofiltration, based on the hypothesis that the EIS system may be used to monitor clotting or clogging phenomena in hemofiltration. The findings suggest that the EIS monitoring system can track changes in ion concentration of blood before and after hemofiltration in real-time, which may serve as an indicator of clogging of filter membranes. Overall, our research demonstrates the potential of TiN-coated tube electrodes for sensitive and versatile non-invasive monitoring in bioreactor cultures and medical devices.
Dieser Band der „Würzburger Studien zur Vor- und Frühgeschichtlichen Archäologie“ legt zum einen die Ergebnisse des DFG-Projektes „Konfliktvermeidung und Konfliktbeilegung in Gesellschaften ohne Zentralgewalt“ vor, das zwischen dem 1. April 2021 und dem 31. März 2024 am Lehrstuhl für Vor- und Frühgeschichtliche Archäologie des Instituts für Altertumswissenschaften der Julius-Maximilians-Universität Würzburg angesiedelt war und unter der Leitung des Herausgebers durchgeführt wurde. Sein einleitender Beitrag gliedert sich nach den drei Themen, die im Rahmen des Projektes bearbeitet wurden. Das erste zielte auf eine begriffliche Klärung des Phänomens der negativen Reziprozität, die nicht als Derivat oder Verfallserscheinung „eigentlicher“ Reziprozität zu verstehen ist, sondern gleichursprünglich mit der positiven in einer strukturellen Reziprozität gründet. Konkret geht es um die Frage, wie Beziehungen des Austauschs wechselseitiger schädigender Handlungen in Gang gesetzt werden, mittels welcher Mechanismen sie auf Dauer gestellt und wie sie gegebenenfalls beendet werden können. Ein wichtiger empirischer Faktor für den Umgang mit Dynamiken negativer Reziprozität ist die Erweiterung der dyadischen Konstellation zu einer triadischen durch Einbeziehung eines Dritten (oder von etwas Drittem). Die gängigen Konfliktmanagementtypologien beschreiben zumeist rollenförmig agierende, unparteiische, personale und singuläre Dritte, deshalb lag der Fokus im Kontext des Projektes auf Konfigurationen von Dritten, die von diesen Typologien invisibilisiert werden, das heißt auf parteiischen oder kollektiv verfassten Dritten sowie tatsächlich unsichtbaren, also imaginierten, fiktiven, abwesenden oder antizipierten Dritten. Und schließlich wurde untersucht, welche Bedeutungen der materiellen Kultur in Prozessen der Konfliktregelung zukommen, welche Funktionen konkrete Objekte übernehmen können bei der Definition und Einrichtung der Situation, der Symbolisierung von Transformationen und Fortschritten in Konfliktlösungsprozessen sowie der sinnfälligen und dauerhaften Symbolisierung der erzielten Ergebnisse.

Introduction:
Fibromyalgia syndrome (FMS) and small fiber neuropathy (SFN) are distinct pain conditions that share commonalities and may be challenging as for differential diagnosis.
Objective:
To comprehensively investigate clinical characteristics of women with FMS and SFN to determine clinically applicable parameters for differentiation.
Methods:
We retrospectively analyzed medical records of 158 women with FMS and 53 with SFN focusing on pain-specific medical and family history, accompanying symptoms, additional diseases, and treatment. We investigated data obtained using standardized pain, depression, and anxiety questionnaires. We further analyzed test results and findings obtained in standardized small fiber tests.
Results:
FMS patients were on average ten years younger at symptom onset, described higher pain intensities requiring frequent change of pharmaceutics, and reported generalized pain compared to SFN. Pain in FMS was accompanied by irritable bowel or sleep disturbances, and in SFN by paresthesias, numbness, and impaired glucose metabolism (P < 0.01 each). Family history was informative for chronic pain and affective disorders in FMS (P < 0.001) and for neurological disorders in SFN patients (P < 0.001). Small fiber pathology in terms of skin denervation and/or thermal sensory threshold elevation was present in 110/158 (69.7 %) FMS patients and 39/53 (73.6 %) SFN patients. FMS patients mainly showed proximally reduced skin innervation and higher corneal nerve branch densities (p<0.001) whereas SFN patients were characterized by reduced cold detection and prolonged electrical A-delta conduction latencies (P < 0.05).
Conclusions:
Our data show that FMS and SFN differ substantially. Detailed pain, drug and family history, investigating blood glucose metabolism, and applying differential small fiber tests may help to improve diagnostic differentiation and targeted therapy.

Hochaufgelöste Erfassung zukünftiger Klimarisiken für Land- und Forstwirtschaft in Unterfranken

Das Klima und seine Veränderungen wirken sich direkt auf die Land- und Forstwirtschaft aus. Daher ist die Untersuchung der zukünftigen Klimarisiken für diese Sektoren von hoher Relevanz. Dies ist auch und vor allem für den schon heute weiträumig trockheitsgeprägten und vom Klimawandel besonders betroffenen nordwestbayerischen Regierungsbezirk Unterfranken der Fall, dessen Gebiet zu über 80 % land- oder forstwirtschaftlich genutzt wird. Zur Untersuchung der Zukunft in hoher räumlicher Auflösung werden Projektionen von regionalen Klimamodellen genutzt. Da diese jedoch Defizite in der Repräsentation des beobachteten Klimas der Vergangenheit aufweisen, sollte vor der weiteren Verwendung eine Anpassung der Daten erfolgen. Dies geschieht in der vorliegenden Arbeit am Beispiel des regionalen Klimamodells REMO im Bezug auf klimatische Kennwerte für Trockenheit, Starkniederschlag, Hitze sowie (Spät-)Frost, die alle eine hohe land- und forstwirtschaftliche Bedeutung besitzen. Die Datenanpassung erfolgt durch zwei verschiedene Ansätze. Zum Einen wird eine Biaskorrektur der aus Globalmodell-angetriebenen REMO-Daten berechneten Indizes durch additive und multiplikative Linearskalierung sowie empirische und parametrische Verteilungsanpassung durchgeführt. Zum Anderen wird ein exploratives Verfahren auf Basis von Model Output Statistics angewandt: Lokale und großräumige atmosphärische Variablen von REMO mit Reanalyseantrieb, die eine zeitliche Korrespondenz zu den Beobachtungen aufweisen, dienen als Prädiktoren für die Aufstellung von Transferfunktionen zur Simulation der Indizes. Diese Transferfunktionen werden sowohl mithilfe Multipler Linearer Regression als auch mit verschiedenen Generalisierten Linearen Modellen konstruiert. Sie werden anschließend genutzt, um Analysen auf Basis von biaskorrigierten Globalmodell-angetriebenen REMO-Prädiktoren durchzuführen. Sowohl für die Biaskorrektur als auch die Model Output Statistics wird eine Kreuzvalidierung durchgeführt, um die Ergebnisse unabhängig vom jeweiligen Trainingszeitraum zu untersuchen und die jeweils besten Varianten zu finden. Werden beide Verfahren mit ihren Unterkategorien für den gesamten historischen Modellzeitraum verglichen, so weist für alle Monat-Kennwert-Kombinationen eine der beiden Verteilungskorrekturen die besten Ergebnisse auf. Die Zukunftsprojektionen unter Verwendung der jeweils erfolgreichsten Methode zeigen im regionalen Durchschnitt für das 21. Jahrhundert negative Trends der (Spät-)Frost- und Eis- sowie positive Trends der Hitzetagehäufigkeit. Winterliche Starkregenereignisse nehmen hinsichtlich ihrer Anzahl zu, im Sommer verstärkt sich die Trockenheit. Die Hinzunahme zwei weiterer regionaler Klimamodelle bestätigt die allgemeinen Zukunftstrends, jedoch ergeben sich beim Spätfrost Widersprüche, wenn dieser hinsichtlich der thermisch abgegrenzten Vegetationsperiode definiert wird.
Zusätzlich werden die Model Output Statistics auf gleiche Weise mit bodennahen Prädiktoren zur Simulation von Erträgen aus Acker- und Weinbau wiederholt. Die Güte kann aufgrund mangelnder Beobachtungsdatenlänge nur anhand der Reanalyse-angetriebenen REMO-Daten abgeschätzt werden, ist hierbei jedoch deutlich besser als im Bezug auf die Kennwertsimulation. Die Zukunftsprojektionen von REMO sowie drei weiterer Regionalmodelle zeigen im Mittel über alle Landkreise Unterfrankens steigende Winter- sowie sinkende Sommerfeldfruchterträge. Hinsichtlich der Frankenweinerträge widersprechen sich die Ergebnisse der drei Klassen Weiß-, Rot- und Gesamtwein insofern, als dass REMO und ein weiteres Modell negative Weiß- und Rotweinertragstrends, jedoch positive Gesamtweinertragstrends simulieren. Die zwei anderen verwendeten Modelle führen durch positive Trendvorzeichen für den Weißwein zu insgesamt kohärenten Ergebnissen.
Die Resultate im Bezug auf die land- und forstwirtschaftlich relevanten klimatischen Kennwerte bedeuten, dass Anpassungsmaßnahmen gegenüber Hitze sowie im Speziellen gegenüber Trockenheit in Zukunft im ohnehin trockenheitsgeprägten Unterfranken an Bedeutung gewinnen werden. Auch die unsicheren Projektionen im Bezug auf die Spätfrostgefahr müssen im Blick behalten werden. Die Trends der Feldfruchterträge deuten in die gleiche Richtung, da Sommergetreide eine höhere Trockenheitsanfälligkeit besitzen. Die unklaren Ergebnisse der Weinerträge hingegen lassen keine eindeutigen Schlüsse zu. Der starke anthropogene Einfluss auf die Erntemengen sowie die großen Unterschiede der Rebsorten hinsichtlich der klimatischen Eignung könnten ein Grund hierfür sein.
RNA viruses rely entirely on the host machinery for their protein synthesis and harbor non-canonical translation mechanisms, such as alternative initiation and programmed –1 ribosomal frameshifting (–1PRF), to suit their specific needs. On the other hand, host cells have developed a variety of defensive strategies to safeguard their translational apparatus and at times transiently shut down global translation. An infection can lead to substantial translational remodeling in cells and translational control is critical during antiviral response. Due to their sheer diversity, this control is likely unique to each RNA virus and the intricacies of post-transcriptional regulation are unclear in certain viral species.
Here, we explored different aspects of translational regulation in virus-infected cells in detail. Using ribosome profiling, we extensively characterized the translational landscape in HIV-1 infected T cells, uncovering novel features of gene regulation in both host and virus. Additionally, we show that substantial pausing occurs prior to the frameshift site indicating complex regulatory mechanisms involving upstream viral RNA elements that can act as cis- regulators of frameshifting.
We also characterized the mechanistic details of trans- modulation of frameshifting by host- and virus-encoded proteins. Host antiviral protein ZAP-S binds to the SARS-CoV-2 frameshift site and destabilizes the stimulatory structure, leading to frameshift inhibition. On the other hand, EMCV 2A protein stabilizes the viral frameshift site, thereby, activating EMCV frameshifting. While both proteins were shown to be antagonistic in their mechanism, they interact with the host translational machinery. Furthermore, we showed that frameshifting can be regulated not just by proteins, but also by small molecules. High-throughput screening of natural and synthetic compounds identified two potent frameshift inhibitors that also impeded viral replication, namely trichangion and compound 25. Together, this work largely enhances our understanding of gene regulation mechanisms in virus-infected cells and further validates the druggability of viral –1 PRF site.
In der vorliegenden Arbeit wurden erstmalig laryngeale Konstriktionen quantitativ und qualitativ in den frühesten Lautäußerungen von Säuglingen mit angeborenen Lippen-Kiefer-Gaumen-Segelspalten (LKGS) in den ersten 3 Lebensmonaten im Längsschnitt untersucht. Als theoretische Grundlage diente Eslings Modell des Laryngealen Artikulators, wonach artikulatorische Entwicklungsvorgänge bereits in den ersten Lebensmonaten der Säuglinge mit der Erzeugung einer Vielzahl laryngealer Lautphänomene beginnen. Potenzielle Einflüsse auf diese präartikulatorischen Vorgänge durch den Ausprägungsgrad der Spaltbildung, das Lebensalter und die kieferorthopädische Frühbehandlung mit Gaumenplatte wurden mittels deskriptiver und interferenzstatistischer Verfahren analysiert.
Von einer sorgfältig ausgewählten Stichprobe, bestehend aus 27 Säuglingen, wurden mehr als 10.000, im Rahmen der interdisziplinären Spaltsprechstunde routinemäßig aufgezeichneten Einzelsignale, retrospektiv untersucht.
Das regelhafte Auftreten von laryngealen Konstriktionen konnte im Untersuchungszeitraum nachgewiesen werden und bestätigt, dass präartikulatorische Übungen primitiver Artikulationsmuster bei Säuglingen mit LKGS in gleicher Weise erfolgen wie bei Säuglingen ohne LKGS.
Die Bedeutung der propriozeptiven Rückkopplung für die Erzeugung von laryngealen Konstriktionen wurde herausgearbeitet und potenzielle marginale Einflüsse der Vokaltraktmalformation diskutiert. Es wird ein früher artikulatorischer Substitutionsmechanismus für die präartikulatorische Entwicklung der Säuglinge mit ausgeprägten Spaltbildungen postuliert.
Hinsichtlich der temporalen Eigenschaften laryngealer Konstriktionen bestätigte ein Vergleich mit Silbenlängen aus der Fachliteratur die Annahme, dass laryngeale Konstriktionen möglicherweise ein konstantes Element zur Rhythmisierung von Lauten im Sinne der artikulatorischen Silbenentwicklung sein könnten.
Acceleration is a central aim of clinical and technical research in magnetic resonance imaging (MRI) today, with the potential to increase robustness, accessibility and patient comfort, reduce cost, and enable entirely new kinds of examinations. A key component in this endeavor is image reconstruction, as most modern approaches build on advanced signal and image processing. Here, deep learning (DL)-based methods have recently shown considerable potential, with numerous publications demonstrating benefits for MRI reconstruction. However, these methods often come at the cost of an increased risk for subtle yet critical errors. Therefore, the aim of this thesis is to advance DL-based MRI reconstruction, while ensuring high quality and fidelity with measured data. A network architecture specifically suited for this purpose is the variational network (VN). To investigate the benefits these can bring to non-Cartesian cardiac imaging, the first part presents an application of VNs, which were specifically adapted to the reconstruction of accelerated spiral acquisitions. The proposed method is compared to a segmented exam, a U-Net and a compressed sensing (CS) model using qualitative and quantitative measures. While the U-Net performed poorly, the VN as well as the CS reconstruction showed good output quality. In functional cardiac imaging, the proposed real-time method with VN reconstruction substantially accelerates examinations over the gold-standard, from over 10 to just 1 minute. Clinical parameters agreed on average.
Generally in MRI reconstruction, the assessment of image quality is complex, in particular for modern non-linear methods. Therefore, advanced techniques for precise evaluation of quality were subsequently demonstrated.
With two distinct methods, resolution and amplification or suppression of noise are quantified locally in each pixel of a reconstruction. Using these, local maps of resolution and noise in parallel imaging (GRAPPA), CS, U-Net and VN reconstructions were determined for MR images of the brain. In the tested images, GRAPPA delivers uniform and ideal resolution, but amplifies noise noticeably. The other methods adapt their behavior to image structure, where different levels of local blurring were observed at edges compared to homogeneous areas, and noise was suppressed except at edges. Overall, VNs were found to combine a number of advantageous properties, including a good trade-off between resolution and noise, fast reconstruction times, and high overall image quality and fidelity of the produced output. Therefore, this network architecture seems highly promising for MRI reconstruction.
Context
Habitat loss and degradation impose serious threats on biodiversity. However, not all habitats receive the attention commensurate with their ecological importance. Shrub ecotones (successional stages between grasslands and forests) can be highly species-diverse but are often restricted to small areas as prevalent management practices either promote open grassland or forest habitats, threatening the effective conservation of ecotone species.
Objectives
In this study, we assessed the importance of habitat and landscape features of shrub ecotones for the rarely studied true bugs (Heteroptera), a functionally diverse taxon that comprises highly specialized species and broad generalists.
Methods
True bugs were sampled with a beating tray in 118 spatially independent shrub ecotones in a region of 45,000 square kilometers in Germany. In addition to habitat area and landscape context, we used a hedge index to evaluate habitat quality.
Results
Shrub ecotones in open habitats harbored a greater species richness and abundance compared to shaded ones in later seral stages, and species composition differed. Richness and abundance were positively affected by increasing habitat area and quality, whereas an increase in the proportion of semi-natural habitats within 1 km only enhanced richness. While feeding and habitat specialists were more sensitive to habitat area reduction than generalists, this was not the case for weak dispersers and carnivores.
Conclusions
Our findings emphasize the importance of large and high-quality ecotones that form a patchy mosaic of shrubs and herbaceous plants. Such ecotones can benefit both grassland species and species depending on woody plants. Conservation authorities should balance between promoting shrubs and keeping such habitats open to maximize species diversity.

Das vorliegende Buch beschäftigt sich anhand einer Sammlung von realen Fällen, die in Aufgabenform formuliert sind, mit dem leider oft gestörten Verhältnis von Theorie und Praxis in der rechtsgeprägten Unternehmensbewertung.

Early-onset torsion dystonia (DYT-TOR1A, DYT1) is an inherited hyperkinetic movement disorder caused by a mutation of the TOR1A gene encoding the torsinA protein. DYT-TOR1A is characterized as a network disorder of the central nervous system (CNS), including predominantly the cortico-basal ganglia-thalamo-cortical loop resulting in a severe generalized dystonic phenotype. The pathophysiology of DYTTOR1A is not fully understood. Molecular levels up to large-scale network levels of the CNS are suggested to be affected in the pathophysiology of DYT-TOR1A. The reduced penetrance of 30% - 40% indicates a gene-environmental interaction, hypothesized as “second hit”. The lack of appropriate and phenotypic DYT-TOR1A animal models encouraged us to verify the “second hit” hypothesis through a unilateral peripheral nerve trauma of the sciatic nerve in a transgenic asymptomatic DYT-TOR1A rat model (∆ETorA), overexpressing the human mutated torsinA protein. In a multiscale approach, this animal model was characterized phenotypically and pathophysiologically.
Nerve-injured ∆ETorA rats revealed dystonia-like movements (DLM) with a partially generalized phenotype. A physiomarker of human dystonia, describing increased theta oscillation in the globus pallidus internus (GPi), was found in the entopeduncular nucleus (EP), the rodent equivalent to the human GPi, of nerve-injured ∆ETorA rats. Altered oscillation patterns were also observed in the primary motor cortex. Highfrequency stimulation (HFS) of the EP reduced DLM and modulated altered oscillatory activity in the EP and primary motor cortex in nerve-injured ∆ETorA rats. Moreover, the dopaminergic system in ∆ETorA rats demonstrated a significant increased striatal dopamine release and dopamine turnover. Whole transcriptome analysis revealed differentially expressed genes of the circadian clock and the energy metabolism, thereby pointing towards novel, putative pathways in the pathophysiology of DYTTOR1A dystonia.
In summary, peripheral nerve trauma can trigger DLM in genetically predisposed asymptomatic ΔETorA rats leading to neurobiological alteration in the central motor network on multiple levels and thereby supporting the “second hit” hypothesis. This novel symptomatic DYT-TOR1A rat model, based on a DYT-TOR1A genetic background, may prove as a valuable chance for DYT-TOR1A dystonia, to further investigate the pathomechanism in more detail and to establish new treatment strategies.
Deep Learning (DL) models are trained on a downstream task by feeding (potentially preprocessed) input data through a trainable Neural Network (NN) and updating its parameters to minimize the loss function between the predicted and the desired output. While this general framework has mainly remained unchanged over the years, the architectures of the trainable models have greatly evolved. Even though it is undoubtedly important to choose the right architecture, we argue that it is also beneficial to develop methods that address other components of the training process. We hypothesize that utilizing domain knowledge can be helpful to improve DL models in terms of performance and/or efficiency. Such model-agnostic methods can be applied to any existing or future architecture. Furthermore, the black box nature of DL models motivates the development of techniques to understand their inner workings. Considering the rapid advancement of DL architectures, it is again crucial to develop model-agnostic methods.
In this thesis, we explore six principles that incorporate domain knowledge to understand or improve models. They are applied either on the input or output side of the trainable model. Each principle is applied to at least two DL tasks, leading to task-specific implementations. To understand DL models, we propose to use Generated Input Data coming from a controllable generation process requiring knowledge about the data properties. This way, we can understand the model’s behavior by analyzing how it changes when one specific high-level input feature changes in the generated data. On the output side, Gradient-Based Attribution methods create a gradient at the end of the NN and then propagate it back to the input, indicating which low-level input features have a large influence on the model’s prediction. The resulting input features can be interpreted by humans using domain knowledge.
To improve the trainable model in terms of downstream performance, data and compute efficiency, or robustness to unwanted features, we explore principles that each address one of the training components besides the trainable model. Input Masking and Augmentation directly modifies the training input data, integrating knowledge about the data and its impact on the model’s output. We also explore the use of Feature Extraction using Pretrained Multimodal Models which can be seen as a beneficial preprocessing step to extract useful features. When no training data is available for the downstream task, using such features and domain knowledge expressed in other modalities can result in a Zero-Shot Learning (ZSL) setting, completely eliminating the trainable model. The Weak Label Generation principle produces new desired outputs using knowledge about the labels, giving either a good pretraining or even exclusive training dataset to solve the downstream task. Finally, improving and choosing the right Loss Function is another principle we explore in this thesis. Here, we enrich existing loss functions with knowledge about label interactions or utilize and combine multiple task-specific loss functions in a multitask setting.
We apply the principles to classification, regression, and representation tasks as well as to image and text modalities. We propose, apply, and evaluate existing and novel methods to understand and improve the model. Overall, this thesis introduces and evaluates methods that complement the development and choice of DL model architectures.
Fusarium (F.)-Infektionen des Auges zeigen oft einen schwerwiegenden Verlauf und sind am häufigsten mit Spezies des Fusarium solani species complex assoziiert. Dabei sind das Tragen von weichen Kontaktlinsen sowie Traumata die wichtigsten prädisponierenden Faktoren. Vorangegangene Untersuchungen des Nationalen Referenzzentrums für invasive Pilzinfektionen hatten ergeben, dass Infektionen durch F. petroliphilum mit der Nutzung von Kontaktlinsen, Infektionen durch F. falciforme jedoch überwiegend traumaassoziiert uns vor allem aus tropischen und subtropischen Ländern bekannt sind.
Das Ziel dieser Arbeit war es daher zu untersuchen, ob F. falcifomre und F. petroliphilum physiologische Merkmale aufweisen, die für die Unterschiede in den Risikofaktoren für Keratitiden durch die beiden Arten verantwortlich sein könnten.
