Background
Juvenile idiopathic arthritis (JIA) is often accompanied by pathomorphological changes to the temporomandibular joint (TMJ). By analyzing orthodontical orthopantomograms of JIA patients the aims of the study were a) classification of condyle changes, b) quantification of bony asymmetries of condylar destruction and c) detection of relationships between disease duration and TMJ-involvement.
Patients/Methods
46 caucasian JIA-patients (28 female; 18 male; < 16.0 years) were enrolled, each joint (n = 92) was morphologically assessed by means of orthopantomogram, quantitatively analysed and compared with duration of general disease. Condyle morphology was assessed using the Billiau scale for severity of destruction [1]. The quantitative analysis was based on ratios of condyle, ramus and mandible height.
Results
Patients were divided into groups (Group I – slightly affected, n = 36; Billiau severity 0–2; condyle findings: X-ray normal, condyle erosions, condylar flattening; Group II – severely affected, N = 10; Billiau severity 3–4; condyle findings: condylar flattenings and erosions, unilateral/bilateral complete loss of condyles), based on morphological analysis of condylar destruction. Duration of disease was significantly longer in Group II (8.9 ± 5.2 years) than in Group I (4.6 ± 4.7 years). Asymmetries of condyle, ramus and mandible height, quantitatively analysed by contralateral comparison, were significantly more marked in patients of Group II than of Group I.
Conclusions
Orthopantomogram imaging can be used in orthodontics clinical routine to detect TMJ-pathologies and is an important reference for monitoring progression of JIA. Classification into severe and slightly affected TMJ is possible by analysis of condylar pathomorphology. An association between degree of destruction, extent of lower jaw asymmetry and disease duration is suggested by the results.
Converging evidence from controlled experiments suggests that the mere processing of a number and its attributes such as value or parity might affect free choice decisions between different actions. For example the spatial numerical associations of response codes (SNARC) effect indicates the magnitude of a digit to be associated with a spatial representation and might therefore affect spatial response choices (i.e., decisions between a "left" and a "right" option). At the same time, other (linguistic) features of a number such as parity are embedded into space and might likewise prime left or right responses through feature words [odd or even, respectively; markedness association of response codes (MARC) effect]. In this experiment we aimed at documenting such influences in a natural setting. We therefore assessed number space and parity space association effects by exposing participants to a fair distribution task in a card playing scenario. Participants drew cards, read out loud their number values, and announced their response choice, i.e., dealing it to a left vs. right player, indicated by Playmobil characters. Not only did participants prefer to deal more cards to the right player, the card's digits also affected response choices and led to a slightly but systematically unfair distribution, supported by a regular SNARC effect and counteracted by a reversed MARC effect. The experiment demonstrates the impact of SNARC- and MARC-like biases in free choice behavior through verbal and visual numerical information processing even in a setting with high external validity.
rOCT1 und hOCT2 sind zwei homologe Transportproteine für organische Kationen, die in Niere und Leber den ersten Schritt der transepithelialen Sekretion von Metaboliten und Xenobiotika vermitteln. Eines ihrer wesentlichen Charakteristika ist neben der Potentialabhängigkeit die Polyspezifität hinsichtlich Transportsubstraten und Hemmstoffen. Beide Transporter können als Uniporter klassifiziert werden, d.h. sie besitzen keine obligate Kopplung an ein Austausch- oder Cosubstrat wie z.B. Natrium, Protonen oder andere organische Kationen. Darüberhinaus zeigen sie das für Transportproteine typische und von Kanälen distinkte Charakteristikum der trans-Stimulierbarkeit. Mit rOCT1 konnten erstmals für einen Prototypen der Transportersuperfamilie SLC22 nähere Aufschlüsse über den Transportmechanismus erhalten werden. Zum einen wurde nachgewiesen, daß rOCT1 eine direktionale Asymmetrie besitzt, d.h. unter Sättigungsbedingungen besteht eine kinetische Bevorzugung der Transportrichtung von extra- nach intrazellulär um den Faktor zwei bis fünf. Zum anderen wurden anhand von rOCT1 neue Erkenntnisse zum Bindungs- und Interaktionsverhalten von Hemmstoffen und Transportsubstraten in Abhängigkeit von der Transportrichtung gewonnen. Hierbei erscheint das bisherige Modell von topologisch festgelegter kompetitiver und allosterischer Hemmung zu stark vereinfacht und nicht zutreffend. Die Bindung von Transportsubstraten und die dadurch induzierten Konformationsänderungen scheinen selbst die Bindungseigenschaften von Hemmstoffen in mehreren Zuständen zu beeinflussen. Auch scheinen Transport- und Ionenleitfähigkeit von OCT zu differenzieren zu sein. Zur Klärung der Fragestellungen wurde das Expressionsmodell Xenopus-Oozyte durch die Etablierung der sogenannten Effluxmethodik funktionell und methodisch wesentlich erweitert.
