Background
Panniculitis-like T-cell lymphoma is an uncommon type of non-Hodgkin lymphoma, occurring usually in the form of nodules within the subcutaneous fat tissue of the extremities or trunk. In the literature, subcutaneous panniculitis-like T-cell lymphoma (SPTCL) is described as a distinct type of T-cell lymphoma with a variable clinical behavior, depending on molecular phenotype of T-cell receptor (TCR) and on the presence or absence of hemophagocytic syndrome.
Case presentation
We present a bioptic and autoptic case of a 65-years old Caucasian man with panniculitic T-cell lymphoma with morphological and immunohistochemical features of SPTCL, limited to the retroperitoneal and mesenteric mass, i.e. without any cutaneous involvement, and associated with severe hemophagocytic lymphohistiocytosis.
Conclusion
A panniculitic T-cell lymphoma with morphological and molecular features of SPTCL, which is limited to mesentery, i.e. does not involve subcutaneous fat, seems to be exceedingly rare.
Bei T-Zell-Lymphomen sind im Gegensatz zu B-Zell-Lymphomen wenige spezifische genetische Aberrationen bekannt. In dieser Arbeit wurden 39 periphere T-Zell-Lymphome (30 PTCL NOS und 9 EATCL) mit der Fluoreszenz-In-Situ-Hybridisierung untersucht. Es wurde ein Screening auf Aberrationen vorgenommen, die zu einem Teil bei B-Zell-Lymphomen eine Rolle spielen, zum anderen bereits für T-Zell-Lymphome beschrieben wurden. Alle untersuchten EATCL wiesen Zugewinne in 9q34 auf. Diese waren signifikant häufiger bei EATCL als bei PTCL NOS. Diese Ergebnisse decken sich mit den CGH-Untersuchungen von Zettl et al.(2002,2004). Ein Vergleich mit Daten aus der Literatur zeigt, daß diese Zugewinne unter peripheren T-Zell-Lymphomen gegenwärtig als spezifisch für EATCL anzusehen sind. Dies ist die erste beschriebene spezifische Aberration bei EATCL. Das Philadelphiachromosom mit der Translokation t(9;22)(q34;q11), das bei der chronisch myeloischen Leukämie gefunden wird, wurde bei EATCL nicht nachgewiesen. Welches das relevante Gen in der Region 9q34 ist, ist noch nicht geklärt. Inwiefern Amplifikationen des NOTCH1-Gen für die Pathogenese der EATCL eine Rolle spielen, müssen weitere Untersuchungen zeigen. Bei PTCL NOS fand sich in 30% der Fälle eine Deletion im langen Arm von Chromosom 5. Dabei war in jedem Fall die Bande 5q21/22 betroffen. Es zeigte sich eine Tendenz zu häufigeren Deletionen von 81c5 als vom proximal gelegenen APC-Gen und in 5q31. Bei den diploiden Fällen war dieser Unterschied statistisch signifikant. Es ist somit anzunehmen, daß der Genlocus distal des APC-Gens und proximal von 5q31 in der Nähe von 81c5 liegt. Verluste in 5q21/22 sind bislang bei T-Zell-Lymphomen nur im Rahmen einer CGH-Studie für PTCL NOS beschrieben worden. Nach unseren Daten scheinen sie bei EATCL keine Rolle zu spielen. Deletionen in 6q21 und von TP53 spielen als sekundäre Aberrationen sowohl bei PTCL NOS als auch bei EATCL eine Rolle. Bei PTCL NOS waren sie signifikant häufiger bei den tetraploiden Fällen zu finden. So waren Deletionen von TP53 bei allen tetraploiden Fällen nachzuweisen. Ein Zusammenhang mit der Progression der Tumoren kann vermuten werden. Dieser Unterschied zwischen diploiden und tetraploiden Fällen fand sich bei EATCL nicht. Aberrationen von ATM, die bei B-Zell-Lymphomen beschrieben wurden, haben bei PTCL NOS und EATCL nach den vorliegenden Daten keine Bedeutung. Bezüglich Deletionen von ATM unterscheiden sich PTCL NOS und EATCL signifikant von T-PLL, bei denen Deletionen von ATM beschrieben wurden. Deletionen von D13S25 wurden zwar sowohl bei PTCL NOS als auch bei EATCL gefunden. Allerdings zeigt ein Vergleich mit der Literatur, dass die minimale überlappende Region der Deletionen eher distal in Bande 13q21 zu suchen ist. Im Rahmen dieser Arbeit konnten erstmals mit der Fluoreszenz-In-Situ-Hybridisierung spezifische Aberrationen bei PTCL NOS und EATCL beschrieben werden. Zudem konnten weitere Aberrationen nachgewiesen werden, die bei diesen Entitäten eine Rolle in der Pathogenese zu spielen scheinen. Weitere Untersuchungen sind notwendig, um ihre Bedeutung in der Diagnostik und Therapie der Malignome zu klären.
Diagnosing any of the more than 30 types of T-cell lymphomas is considered a challenging task for many pathologists and currently requires morphological expertise as well as the integration of clinical data, immunophenotype, flow cytometry and clonality analyses. Even considering all available information, some margin of doubt might remain using the current diagnostic procedures. In recent times, the genetic landscape of most T-cell lymphomas has been elucidated, showing a number of diagnostically relevant mutations. In addition, recent data indicate that some of these genetic alterations might bear prognostic and predictive value. Extensive genetic analyses, such as whole exome or large panel sequencing are still expensive and time consuming, therefore limiting their application in routine diagnostic. We therefore devoted our effort to develop a lean approach for genetic analysis of T-cell lymphomas, focusing on maximum efficiency rather than exhaustively covering all possible targets. Here we report the results generated with our small amplicon-based panel that could be used routinely on paraffin-embedded and even decalcified samples, on a single sample basis in parallel with other NGS-panels used in our routine diagnostic lab, in a relatively short time and with limited costs. We tested 128 available samples from two German reference centers as part of our routine work up (among which 116 T-cell lymphomas), which is the largest routine diagnostic series reported to date. Our results showed that this assay had a very high rate of technical success (97%) and could detect mutations in the majority (79%) of tested T-cell lymphoma samples.
