Background
Troponin elevation is common in ischemic stroke (IS) patients. The pathomechanisms involved are incompletely understood and comprise coronary and non-coronary causes, e.g. autonomic dysfunction. We investigated determinants of troponin elevation in acute IS patients including markers of autonomic dysfunction, assessed by heart rate variability (HRV) time domain variables.
Methods
Data were collected within the Stroke Induced Cardiac FAILure (SICFAIL) cohort study. IS patients admitted to the Department of Neurology, Würzburg University Hospital, underwent baseline investigation including cardiac history, physical examination, echocardiography, and blood sampling. Four HRV time domain variables were calculated in patients undergoing electrocardiographic Holter monitoring. Multivariable logistic regression with corresponding odds ratios (OR) and 95% confidence intervals (CI) was used to investigate the determinants of high-sensitive troponin T (hs-TnT) levels ≥14 ng/L.
Results
We report results from 543 IS patients recruited between 01/2014–02/2017. Of those, 203 (37%) had hs-TnT ≥14 ng/L, which was independently associated with older age (OR per year 1.05; 95% CI 1.02–1.08), male sex (OR 2.65; 95% CI 1.54–4.58), decreasing estimated glomerular filtration rate (OR per 10 mL/min/1.73 m2 0.71; 95% CI 0.61–0.84), systolic dysfunction (OR 2.79; 95% CI 1.22–6.37), diastolic dysfunction (OR 2.29; 95% CI 1.29–4.02), atrial fibrillation (OR 2.30; 95% CI 1.25–4.23), and increasing levels of C-reactive protein (OR 1.48 per log unit; 95% CI 1.22–1.79). We did not identify an independent association of troponin elevation with the investigated HRV variables.
Conclusion
Cardiac dysfunction and elevated C-reactive protein, but not a reduced HRV as surrogate of autonomic dysfunction, were associated with increased hs-TnT levels in IS patients independent of established cardiovascular risk factors.
Background: Phosphodiesterases (PDE) critically regulate myocardial cAMP and cGMP levels. PDE2 is stimulated by cGMP to hydrolyze cAMP, mediating a negative crosstalk between both pathways. PDE2 upregulation in heart failure contributes to desensitization to β-adrenergic overstimulation. After isoprenaline (ISO) injections, PDE2 overexpressing mice (PDE2 OE) were protected against ventricular arrhythmia. Here, we investigate the mechanisms underlying the effects of PDE2 OE on susceptibility to arrhythmias. Methods: Cellular arrhythmia, ion currents, and Ca\(^{2+}\)-sparks were assessed in ventricular cardiomyocytes from PDE2 OE and WT littermates. Results: Under basal conditions, action potential (AP) morphology were similar in PDE2 OE and WT. ISO stimulation significantly increased the incidence of afterdepolarizations and spontaneous APs in WT, which was markedly reduced in PDE2 OE. The ISO-induced increase in I\(_{CaL}\) seen in WT was prevented in PDE2 OE. Moreover, the ISO-induced, Epac- and CaMKII-dependent increase in I\(_{NaL}\) and Ca\(^{2+}\)-spark frequency was blunted in PDE2 OE, while the effect of direct Epac activation was similar in both groups. Finally, PDE2 inhibition facilitated arrhythmic events in ex vivo perfused WT hearts after reperfusion injury. Conclusion: Higher PDE2 abundance protects against ISO-induced cardiac arrhythmia by preventing the Epac- and CaMKII-mediated increases of cellular triggers. Thus, activating myocardial PDE2 may represent a novel intracellular anti-arrhythmic therapeutic strategy in HF.
Acute and chronic cardiac disorders predispose to alterations in cognitive performance, ranging from mild cognitive impairment to overt dementia. Although this association is well-established, the factors inducing and accelerating cognitive decline beyond ageing and the intricate causal pathways and multilateral interdependencies involved remain poorly understood. Dysregulated and persistent inflammatory processes have been implicated as potentially causal mediators of the adverse consequences on brain function in patients with cardiac disease. Recent advances in positron emission tomography disclosed an enhanced level of neuroinflammation of cortical and subcortical brain regions as an important correlate of altered cognition in these patients. In preclinical and clinical investigations, the thereby involved domains and cell types of the brain are gradually better characterized. Microglia, resident myeloid cells of the central nervous system, appear to be of particular importance, as they are extremely sensitive to even subtle pathological alterations affecting their complex interplay with neighboring astrocytes, oligodendrocytes, infiltrating myeloid cells, and lymphocytes. Here, we review the current evidence linking cognitive impairment and chronic neuroinflammation in patients with various selected cardiac disorders including the aspect of chronic neuroinflammation as a potentially druggable target.
Diese retrospektive Auswertung von PatientInnendaten der kardiologischen Ambulanz des Universitätsklinikums Würzburg konnte zeigen, dass die Bestimmung der diastolischen Dysfunktion prognostisch relevante Informationen enthält. Das Studienkollektiv wurde anhand der gemessenen Ejektionsfraktion in die zwei Untersuchungsgruppen HFrEF und HFmrEF eingeteilt. Diese zwei Untersuchungsgruppen wurden anhand ihrer klinisch und echokardiographisch bestimmten Charakteristika verglichen. Anschließend wurden drei diastolische Parameter (E/e’, LAVi und TRVmax) auf ihre prognostische Relevanz untersucht. Die abschließende Untersuchung gruppierte die PatientInnen anhand der Schwere ihrer diastolischen Dysfunktion (mild / moderat / schwer) und untersuchte ebenfalls das Langzeitüberleben.
Die HFmrEF-Gruppe zeigte ähnliche klinische Charakteristika wie die HFrEF-Gruppe. Eine ischämische Genese der Herzinsuffizienz wurde in der HFmrEF-Gruppe im Vergleich zur HFrEF-Gruppe häufiger beobachtet.
Die Überlebenszeitanalysen konnten bei PatientInnen in der HFmrEF-Gruppe zeigen, dass ein dilatierter linker Vorhof (LAVi) oder eine große Regurgitation über der Trikuspidalklappe (TRVmax) mit einer schlechten Prognose einhergehen. Bei HFrEF-PatientInnen hingegen konnte dies nicht nachgewiesen werden. Hier zeigte sich, dass insbesondere der Parameter E/e’septal prognostisch relevante Informationen enthält.
Die Auswertung der Untersuchungsgruppen nach Einteilung anhand der Schwere der diastolischen Dysfunktion konnte die gefunden Effekte bestätigen. Eine moderate bis schwere diastolische Dysfunktion war mit einer signifikant schlechteren Prognose behaftet, und zwar sowohl in der HFrEF- wie auch in der HFmrEF-Gruppe.
Die gefunden Ergebnisse zeigen, dass die diastolische Dysfunktion auch bei PatientInnen mit einer systolischen Herzinsuffizienz wichtige prognostische Informationen enthalten. In der klinischen Routine sollte die echokardiographische Bestimmung der diastolischen Herzfunktion standardmäßig durchgeführt werden.
Die Ergebnisse könnten nicht nur in der Diagnostik zur Kategorisierung der PatientInnen und Bestimmung der Prognose, sondern auch hinsichtlich der Therapie von großem zukünftigem Nutzen sein. Hierzu sollten perspektivisch vor allem therapeutische Aspekte in prospektiven, idealerweise randomisierten Studien untersucht werden, welche sich auf die Erkenntnisse dieser Arbeit beziehen.
Die Prävalenz der chronisch obstruktiven pulmonalen Erkrankung (COPD) bei der systolischen Herzinsuffizienz (HF) wird in der Literatur mit bis zu 20-30% angegeben. Die meisten Publikationen leiteten die Diagnose einer COPD aus anamnestischen Angaben ab, die tatsächliche Prävalenz der spirometrisch bestätigten COPD bei komorbider Herzinsuffizienz war bislang unklar. Ziel der vorliegenden Arbeit war es, die Anteile der Patienten mit anamnestischer COPD sowie mit spirometrisch verifizierter Obstruktiver Ventilationsstörung (OVS) bei einer großen Kohorte von Patienten mit systolischer Herzinsuffizienz zu ermitteln, Charakteristika beider Patientengruppen miteinander zu vergleichen und geschlechtsspezifische Unterschiede aufzudecken. Im Rahmen einer retrospektiven Substudie der INH (Interdisziplinäres Netzwerk Herzinsuffizienz) - Studie wurden Lungenfunktionsanalysen, echokardiographische und anamnestische Befunde von insgesamt 632 Patienten ausgewertet. Die Patienten stellten sich hierzu 6 Monate nach stationärer Behandlung, die aufgrund einer kardialen Dekompensation erfolgt war, in der Herzinsuffizienzambulanz der Universitätsklinik vor. Die Prävalenz der anamnestischen COPD lag in unserem Kollektiv bei 24%. Spirometrisch liess sich eine Obstruktion aber nur bei 16% aller Patienten (98/632) bestätigen. Der positiv prädiktive Wert der anamnestischen COPD zur Vorhersage der spirometrisch bestätigen Atemwegsobstruktion lag mit 0,32 noch unterhalb der zufälligen Ratewahrscheinlichkeit von 0,5. Damit war die anamnestische COPD in 68% der Fälle nicht zutreffend diagnostiziert. Eine COPD konnte zudem bei mehr als der Hälfte aller Patienten mit anamnestischer COPD (83/151, entsprechend 55%), spirometrisch ausgeschlossen werden. Bei Patientinnen mit anamestischer COPD war eine häufigere Einnahme von Schleifendiuretika bei vermehrtem Auftreten klinischer Stauungszeichen zu verzeichnen. Ferner erhielten Frauen (72 vs. 89%, P<0,01), nicht jedoch Männer mit OVS weniger häufig einen Betablocker, was sowohl als Ursache als auch als Konsequenz einer häufigeren (subklinischen) kardialen Dekompensation angesehen werden könnte. Weitere Hinweise darauf, dass v.a. bei Frauen oft keine „echte“, auf Nikotinabusus zurückzuführende COPD vorlag, finden sich bei der Betrachtung des Nichtraucheranteils bei der geschlechtsvergleichenden Analyse Frauen vs. Männer (66,7 vs. 26,5; P<0,01) bzw. bei der Subgruppenanalyse Frauen mit vs. ohne OVS (72,7 vs. 73,5; P=0,57). Die Komorbidität von OVS und Herzinsuffzienz hatte hingegen bei Männern besonders auf die physische Komponente der Lebensqualität negative Auswirkungen (SF 36 Physische Funktion Männer mit vs. ohne OVS 50 vs. 62, P<0,01). In einer Subgruppenanalyse von 278 Patienten, die aufgrund einer kardialen Dekompensation stationär behandelt wurden, und von denen bei 52 denen während des Aufenthaltes spirometrisch eine OVS festgestellt wurde, war die Obstruktion 6 Monate nach Entlassung bei der Hälfte der Patienten nicht mehr nachweisbar. In der multivariaten logistischen Regression waren nur bodyplethysmographischen Parameter wie das Residualvolumen (RLV), das Intrathorakale Gasvolumen (ITGV) und die Totale Lungenkapazität (TLC), nicht jedoch die spirometrischen Messparameter FEV1, FVC und FEV1/FVC prädiktiv für die Persistenz der OVS 6 Monate nach Entlassung. Unsere Daten belegen, dass bei Herzinsuffizienz die anamnestische COPD nur unzureichend mit der spirometrisch nachgewiesenen Obstruktion korreliert. Die COPD bei Herzinsuffizienz wird häufig überschätzt und somit ohne Indikation mit inhalativen Antiobstruktiva therapiert. Auch kommen Betablocker entgegen den Empfehlungen der Leitlinien bei Vorhandensein einer obstruktiven Ventilationsstörung sowie einer Herzinsuffizienz bei Frauen seltener zum Einsatz. Oft ist eine kardiale Stauung und nicht die COPD Ursache einer nachweisbaren Obstruktion. Zur korrekten Diagnose bei OVS und vermuteter kardialer Stauung sind Parameter der Bodyplethysmographie wie das RLV oder ITGV, die bei Erhöhung einen verlässlichen Hinweis auf eine zugrunde liegende COPD liefern, hilfreich. Nach diesen Daten erscheint bei Herzinsuffizienzpatienten mit Verdacht auf eine obstruktive Lungenerkrankung nach bestmöglicher Rekompensation und vor Beginn einer medikamentösen anti-obstruktiven Therapie die Durchführung einer Lungenfunktionsuntersuchung mit Spirometrie und Bodyplethysmographie grundsätzlich indiziert.
Die Na+ /K+ -ATPase (NKA) ist maßgeblich an der Regulation der kardialen Na+ -Homöostase beteilligt. Im Myokard werden hauptsächlich zwei Isoformen exprimiert: die α1 (NKA-α1) und die α2-Isoform (NKA-α2). Diese beiden Isoformen unterscheiden sich sowohl in ihrer Lokalisation als auch in ihrer zellulären Funktion. So ist die NKA-α1 recht homogen entlang des Sarkolemms zu finden und ist verantwortlich für die Regulation der globalen intrazellulären Na+ -Konzentration ([Na+ ]i). Die NKA-α2 hingegen konzentriert sich hauptsächlich in den T-Tubuli und beeinflusst über Veränderung der lokalen [Na+ ]i die Ca2+ -Transienten und die Kontraktilität. Im Rahmen einer Herzinsuffizienz wurde eine verminderte Expression und Aktivität der NKA beobachtet. Gleichzeitig werden Inhibitoren der NKA, sogenannte Digitalisglykoside, in fortgeschrittenen Herzinsuffizienz-Stadien eingesetzt. Die Studienlage über den Einsatz dieser Therapeutika ist recht uneinheitlich und reicht von einer verringerten Hospitalisierung bis hin zu einer erhöhten Mortalität. Ziel dieser Arbeit war es die Folgen einer NKA-α2 Aktivierung während einer Herzinsuffizienz mit Hilfe eines murinen Überexpressionsmodells zu analysieren. 11-Wochen alte Mäuse mit einer kardialen NKA-α2 Überexpression (NKA-α2) und Wildtyp (WT) Versuchstiere wurden einem 8-wöchigen Myokardinfarkt (MI) unterzogen. NKA-α2 Versuchstiere waren vor einem pathologischem Remodeling und einer kardialen Dysfunktion geschützt. NKA-α2 Kardiomyozyten zeigten eine erhöhte Na+ /Ca2+ -Austauscher (NCX) Aktivität, die zu niedrigeren diastolischen und systolischen Ca2+ -Spiegeln führte und einer Ca2+ -Desensitisierung der Myofibrillen entgegenwirkte. WT Versuchstiere zeigten nach chronischem MI eine sarkoplasmatische Ca2+ -Akkumulation, die in NKA-α2 Kardiomyozyten ausblieb. Gleichzeitig konnte in der NKA-α2 MI Kohorte im Vergleich zu den WT MI Versuchstieren eine erhöhte Expression von β1-adrenergen Rezeptoren (β1AR) beobachtet werden, die eine verbesserte Ansprechbarkeit gegenüber β-adrenergen Stimuli bewirkte. Zudem konnte in unbehandelten Versuchstieren eine Interaktion zwischen NKA-α2 und dem β1AR nachgewiesen werden, welche in der WT Kohorte größer ausfiel als in der NKA-α2 Versuchsgruppe. Gleichzeitig zeigten unbehandelte NKA-α2 Kardiomyozyten eine erhöhte Sensitivität gegenüber β-adrenerger Stimulation auf, welche nicht mit einer erhöhten Arrhythmie-Neigung oder vermehrten Bildung reaktiver Sauerstoffspezies einherging. Diese Untersuchungen zeigen, dass eine NKA-α2 Überexpression vor pathologischem Remodeling und einer kardialen Funktionbeeinträchtigung schützt, indem eine systolische, diastolische und sarkoplasmatische Ca2+ -Akkumulation verhindert wird. Gleichzeitig wird die β1AR Expression stabilisert, wodurch es zu einer verminderten neurohumoralen Aktivierung und einer Durchbrechung des Circulus vitiosus kommen könnte. Insgesamt scheint eine Aktivierung der NKA-α2 durchaus ein vielversprechendes Target in der Herzinsuffizienz Therapie darzustellen.
Therapie darzustellen.
Herzinfarkt und seine Folgen, insbesondere die Herzhypertrophie und die daraus resultierende Herzinsuffizienz stellen trotz großer Fortschritte in der Therapie immer noch die Haupttodesursache in den Industrieländern dar. Man hat in den letzten Jahren viele Pathomechanismen der Entstehung der Hypertrophie entdeckt. Seitdem es möglich ist, die molekularen Vorgänge einzelner Zellen zu erforschen, hat sich ein Augenmerk der Zellforschung auf enzymatische Signalwege in Zellen gelegt. Man konnte feststellen, dass das Wachstum einer Zelle auf der Induktion verschiedenster Enzymkaskaden unter Beteiligung mannigfaltiger Kinasen und Proteine beruht. Die einzelnen Schritte sind weitestgehend ungeklärt und vor allem die Kooperation und das Zusammenwirken einzelner Auslöser und Schlüsselenzyme sind bisher nur in groben Zügen bekannt. Eines der Schlüsselproteine im Zellwachstum und in der Übermittlung von Informationen von Zelle zu Zelle und Zellumgebung in das Zellinnere und umgekehrt stellt die Gruppe der Integrine dar. Die Beteiligung dieser bei verschiedensten physiologischen Prozessen wie Organwachstum, Angiogenese und Adaption an neue Anforderungen an den Körper wurde nachgewiesen. Sie stehen in enger Kooperation mit anderen intrazellulären und extrazellulären Proteinen und leiten Signale über Kinasen wie beispielsweise die Tyrosinkinasen FAK und Src weiter und stellen somit eine Schaltzentrale zellulärer Adaption dar. Dies trifft auch auf zahlreiche pathologische Prozesse zu,so zum Beispiel Neoplasien und eben Herzhypertrophie. Man hat in zahlreichen Forschungsprojekten den Zusammenhang von Integrinen und ihren Subtypen mit der Entstehung von Herzhypertrophie festgestellt. Dies betraf bisher vor allem die Integrine β1 und β3. In dieser Arbeit nun konnte ein enger Zusammenhang von Herzhypertrophie im Rahmen von Herzinfarkt am Mausmodel und dem Integrin αv festgestellt werden. Dies konnte durch direkte Untersuchungen in vivo mittels Herzecho als auch in vitro mittels Histologie am Gewebe gezeigt werden, da eine Hypertrophie nur bei den Tieren vorkam, die keinen Integrin-αv-Antagonisten verabreicht bekommen hatten. Die Gruppe ohne funktionsfähiges Integrin αv dagegen entwickelte keine Adaption an die veränderten pathologischen Verhältnisse und hypertrophierte nicht. Weiterhin konnte man die molekularen Veränderungen und die intrazelluläre Signalverarbeitung im Western Blot und an Gefrierschnitten darstellen und zeigen, dass die Aktivierung von Src, FAK und ERK-1/2-Kinase bei integrin-αvβ5-inhibierten Tieren ausbleibt. Da gleichzeitig bei den Tieren mit funktionstüchtigem Integrin β5 eine Zunahme phosphorylierter Kinasen nachgewiesen werden konnte, muss man von einer entscheidenden Rolle dieses Integrins an der Weiterleitung von Signalen im hypertrophierenden Herz ausgehen. Integrin αv hat nach diesen Ergebnissen einen mitentscheidenden Einfluss auf die Entstehung von Herzhypertrophie nach Myokardinfarkt. Dies bietet neue Chancen und Alternativen in der Therapie von Folgeschäden nach Myokardinfarkt. Das Ausschalten von αv führte zu einer erhöhten Mortalität der Tiere mit Infarkt. Allerdings war das zu erwarten, da jedwede Kompensierung im kranken Herzen ausblieb. Eine gewisse konsekutive Herzwandverdickung jedoch ist erwünscht (LaPlace) und für das Überleben notwendig. Ebenso konnte man erkennen, dass die Substanz bei gesunden Mäusen keinen Schaden anrichtet. Sie scheint sogar einen positiven Effekt auf die Pumpleistung des Herzens zu haben. Als neue Untersuchung wäre es daher empfehlenswert, den Tieren die Substanz erst zu verabreichen, wenn die Akutphase des Myokardinfarktes überstanden ist und ein gewisser Hypertrophieprozess eingetreten ist. Es ist durchaus plausibel, davon auszugehen, dass die zu einem späteren Zeitpunkt verabreichte Substanz in der Lage sein könnte, eine übermäßige Hypertrophie zu vermeiden. Dies würde bedeuten, dass man eine neuartige Medikation entwickeln könnte, die einen wichtigen Basispfeiler zur Vermeidung von chronischem Herzversagen nach Herzinfarkt darstellen könnte. Mit gezielter Hemmung einzelner Wachstumsstimulatoren von Zellen wäre man in der Lage, Zellwachstum gezielt zu regulieren, und somit eine physiologische Adaption an pathologische Prozesse zu erreichen
Diabetes mellitus und Herzinsuffizienz gehen mit einer stark erhöhten kardiovaskulären Morbidität einher. Bei beiden Krankheiten besteht eine Endotheldysfunktion und eine verstärkte Thrombozytenaktivierung. Daraus resultieren wiederum frühe atherosklerotische Läsionen bzw. eine Progression der Herzinsuffizienz. Das Chemokin Fractalkine wurde als Risikofaktor für eine schwere koronare Herzerkrankung beschrieben. Der Fractalkine-Rezeptor ist mit einem erhöhten Atherosklerose-Risiko assoziiert. Auch ist Fractalkine in der Lage, eine Endotheldysfunktion sowie eine Thrombozytenaktivierung zu induzieren bzw. zu verstärken. Es lag daher nahe, den Effekt von Fractalkine auf Endothel und Thrombozyten von Ratten mit Herzinsuffizienz und Diabetes mellitus zu untersuchen. Die Expression von Fractalkine wie auch seines Rezeptors war in der Aorta bei Herzinsuffizienz und Diabetes mellitus gesteigert. Auch das lösliche Fractalkine im Serum war bei beiden Krankheitsmodellen erhöht. Auf der Thrombozytenoberfläche zeigte sich eine stärkere Fractalkine-Rezeptor-Expression. Durch Stimulation mit Fractalkine konnte bei beiden Modellen eine signifikant akzelerierte Thrombozytenaktivierung erzielt werden. Bei den herzinsuffizienten Tieren zeigte sich eine signifikante Verschlechterung der schon bestehenden endothelialen Dysfunktion nach Inkubation mit Fractalkine. Deshalb kann man davon ausgehen, dass Fractalkine eine bedeutende Rolle sowohl für die Atherosklerose bei Diabetes mellitus als auch bei Herzinsuffizienz spielt. Man könnte sich nun Fractalkine und seinen Rezeptor als einen neuen therapeutischen Ansatzpunkt für die Verhinderung der Progression sowohl atherosklerotischer Läsionen als auch der Inflammation bei Herzinsuffizienz vorstellen. Dadurch wäre eine Reduzierung weiterer atherosklerotischer Komplikationen wie Myokardinfarkt oder Schlaganfall eventuell möglich.
Histone H3 serine 28 (H3S28) phosphorylation and de-repression of polycomb repressive complex (PRC)-mediated gene regulation is linked to stress conditions in mitotic and post-mitotic cells. To better understand the role of H3S28 phosphorylation in vivo, we studied a Drosophila strain with ectopic expression of constitutively-activated H3S28A, which prevents PRC2 binding at H3S28, thus mimicking H3S28 phosphorylation. H3S28A mutants showed prolonged life span and improved resistance against starvation and paraquat-induced oxidative stress. Morphological and functional analysis of heart tubes revealed smaller luminal areas and thicker walls accompanied by moderately improved cardiac function after acute stress induction. Whole-exome deep gene-sequencing from isolated heart tubes revealed phenotype-corresponding changes in longevity-promoting and myotropic genes. We also found changes in genes controlling mitochondrial biogenesis and respiration. Analysis of mitochondrial respiration from whole flies revealed improved efficacy of ATP production with reduced electron transport-chain activity. Finally, we analyzed posttranslational modification of H3S28 in an experimental heart failure model and observed increased H3S28 phosphorylation levels in HF hearts. Our data establish a critical role of H3S28 phosphorylation in vivo for life span, stress resistance, cardiac and mitochondrial function in Drosophila. These findings may pave the way for H3S28 phosphorylation as a putative target to treat stress-related disorders such as heart failure.
Thiazolidindione, die zunehmend als gut wirkende Insulinsensitizer in der Diabetestherapie im Einsatz sind, besitzen als indikationslimitierende Nebenwirkung eine starke Flüssigkeitsretention mit Kontraindikation bei Herzinsuffizienz. Andererseits wird ihnen in der derzeitigen experimentellen Studienlage auf Zellebene ein positiver Effekt auf kardiale und vaskulär bedingte Erkrankungen zugesprochen. In der vorliegenden Arbeit wird die Hypothese untersucht, inwieweit Pioglitazon einen positiven oder negativen Einfluss auf das Remodeling nach Myokardinfarkt hat und ob sich Folgen für die Entwicklung einer Herzinsuffizienz ergeben. Dazu wird eine Untersuchung an Mäusen nach Myokardinfarktoperation unter Pioglitazonbehandlung, (ab dem siebten Tag nach OP) und einer entsprechenden Sham- Kontrollgruppe durchgeführt. Die Verabreichung von Pioglitazon versus Placebo erfolgt täglich körpergewichtsbezogen per Schlundsonde. Im Verlauf der Studie werden die Tiere am siebten, 21. und 42. Tag in apikaler Ebene und in Höhe des Papillarmuskels echokardiographiert und die Daten als M- und B-Mode aufgezeichnet und ausgewertet. Weiterhin wird das Gewicht der Tiere am Operationstag und nachfolgend wöchentlich erfasst. Nach Studienende werden die entfernten Herzen der Tiere gewogen sowie der Glucose- und GOT-Wert des Blutes erfasst. Weiterhin erfolgt die Messung der Aortenrelaxation, die Infarktgrößenbestimmung und Kollagenmessung sowie die Bestimmung von TNFα, NF-κB, IL-1β und Endothelin-1. Wie erwartet, kann infarktbedingt eine Dilatation des Ventrikels und eine Zunahme des Kollagengehaltes echokardiographisch und polarisationsmikroskopisch dokumentiert werden. Vergleichend lassen sich weder bezüglich des Gewichtes der Herzen und der Tiere, der Myokardinfarktgröße, des Kollagengehalts im gesunden und infarzierten Myokardgewebe, des Remodeling, der proinflammatorischen Enzyme und Endothelin-1, noch in der Gefäßreaktion signifikante Unterschiede feststellen. Während der Serumglukosewert bei den verwendeten nicht an Diabetes mellitus erkrankten Tieren keinen Unterschied zwischen den beiden Behandlungsgruppen zeigt, lässt sich in der pioglitazon©behandelten Gruppe eine deutliche Senkung der Triglyceridspiegel feststellen. Auf Basis der vorliegenden Messungen zeigt die Pioglitazonbehandlung keinen positiven oder negativen Effekt auf das Remodeling von infarzierten Mäusen.
Establishing a cardiac training group for patients with heart failure: the "HIP-in-Würzburg" study

Background
Exercise training in heart failure (HF) is recommended but not routinely offered, because of logistic and safety-related reasons. In 2020, the German Society for Prevention&Rehabilitation and the German Society for Cardiology requested establishing dedicated ""HF training groups."" Here, we aimed to implement and evaluate the feasibility and safety of one of the first HF training groups in Germany.
Methods
Twelve patients (three women) with symptomatic HF (NYHA class II/III) and an ejection fraction ≤ 45% participated and were offered weekly, physician-supervised exercise training for 1 year. Patients received a wrist-worn pedometer (M430 Polar) and underwent the following assessments at baseline and after 4, 8 and 12 months: cardiopulmonary exercise test, 6-min walk test, echocardiography (blinded reading), and quality of life assessment (Kansas City Cardiomyopathy Questionnaire, KCCQ).
Results
All patients (median age [quartiles] 64 [49; 64] years) completed the study and participated in 76% of the offered 36 training sessions. The pedometer was worn ≥ 1000 min per day over 86% of the time. No cardiovascular events occurred during training. Across 12 months, NT-proBNP dropped from 986 pg/ml [455; 1937] to 483 pg/ml [247; 2322], and LVEF increased from 36% [29;41] to 41% [32;46]%, (p for trend = 0.01). We observed no changes in exercise capacity except for a subtle increase in peak VO2% predicted, from 66.5 [49; 77] to 67 [52; 78]; p for trend = 0.03. The physical function and social limitation domains of the KCCQ improved from 60 [54; 82] to 71 [58; 95, and from 63 [39; 83] to 78 [64; 92]; p for trend = 0.04 and = 0.01, respectively. Positive trends were further seen for the clinical and overall summary scores.
Conclusion
This pilot study showed that the implementation of a supervised HF-exercise program is feasible, safe, and has the potential to improve both quality of life and surrogate markers of HF severity. This first exercise experiment should facilitate the design of risk-adopted training programs for patients with HF.
Cancer and heart disease are leading causes of morbidity and mortality worldwide. These diseases have common risk factors, common molecular signaling pathways that are central to their pathogenesis, and even some disease phenotypes that are interdependent. Thus, a detailed understanding of common regulators is critical for the development of new and synergistic therapeutic strategies. The Raf kinase inhibitory protein (RKIP) is a regulator of the cellular kinome that functions to maintain cellular robustness and prevent the progression of diseases including heart disease and cancer. Two of the key signaling pathways controlled by RKIP are the β-adrenergic receptor (βAR) signaling to protein kinase A (PKA), particularly in the heart, and the MAP kinase cascade Raf/MEK/ERK1/2 that regulates multiple diseases. The goal of this review is to discuss how we can leverage RKIP to suppress cancer without incurring deleterious effects on the heart. Specifically, we discuss: (1) How RKIP functions to either suppress or activate βAR (PKA) and ERK1/2 signaling; (2) How we can prevent cancer-promoting kinase signaling while at the same time avoiding cardiotoxicity.
Background:
Adherence to pharmacotherapeutic treatment guidelines in patients with heart failure (HF) is of major prognostic importance, but thorough implementation of guidelines in routine care remains insufficient. Our aim was to investigate prevalence and characteristics of HF in patients with coronary heart disease (CHD), and to assess the adherence to current HF guidelines in patients with HF stage C, thus identifying potential targets for the optimization of guideline implementation.
Methods:
Patients from the German sample of the European Action on Secondary and Primary Prevention by Intervention to Reduce Events (EuroAspire) IV survey with a hospitalization for CHD within the previous six to 36 months providing valid data on echocardiography as well as on signs and symptoms of HF were categorized into stages of HF: A, prevalence of risk factors for developing HF; B, asymptomatic but with structural heart disease; C, symptomatic HF. A Guideline Adherence Indicator (GAI-3) was calculated for patients with reduced (≤40%) left ventricular ejection fraction (HFrEF) as number of drugs taken per number of drugs indicated; beta-blockers, angiotensin converting enzyme inhibitors/angiotensin receptor blockers, and mineralocorticoid receptor antagonists (MRA) were considered.
Results:
509/536 patients entered analysis. HF stage A was prevalent in n = 20 (3.9%), stage B in n = 264 (51.9%), and stage C in n = 225 (44.2%) patients; 94/225 patients were diagnosed with HFrEF (42%). Stage C patients were older, had a longer duration of CHD, and a higher prevalence of arterial hypertension. Awareness of pre-diagnosed HF was low (19%). Overall GAI-3 of HFrEF patients was 96.4% with a trend towards lower GAI-3 in patients with lower LVEF due to less thorough MRA prescription.
Conclusions:
In our sample of CHD patients, prevalence of HF stage C was high and a sizable subgroup suffered from HFrEF. Overall, pharmacotherapy was fairly well implemented in HFrEF patients, although somewhat worse in patients with more reduced ejection fraction. Two major targets were identified possibly suited to further improve the implementation of HF guidelines: 1) increase patients´ awareness of diagnosis and importance of HF; and 2) disseminate knowledge about the importance of appropriately implementing the use of mineralocorticoid receptor antagonists.
Trial registration:
This is a cross-sectional analysis of a non-interventional study. Therefore, it was not registered as an interventional trial.
Herzinsuffizienz, Depression und Angststörungen treten gehäuft gemeinsam auf und beeinflussen teilweise gegenseitig ihre Prognose. Die Zusammenhänge zwischen diesen Erkrankungen sind bislang nicht aufgeklärt.
In der vorliegenden Arbeit führte ischämische Herzinsuffizienz im Mausmodell zu Depressions-ähnlichem Verhalten innerhalb von 8 Wochen nach Infarktinduktion. Weiter zeigte sich eine Minderung der Gedächtnisleistung. Angst-assoziiertes Verhalten ließ sich nicht nachweisen. Immunhistochemisch konnten keine Veränderungen in spezifischen Hirnarealen nachgewiesen werden. Molekulare Methoden legen Veränderungen des Serotoninstoffwechsels als mögliche Erklärung nahe.
Nach operativer Ligatur eines Herzkrankgefäßes wurden C57/Bl6N Mäuse über einen Zeitraum von 8 Wochen beobachtet. In dieser Zeit wurden neben Herzultraschalluntersuchungen eine Reihe von Verhaltenstest durchgeführt, um depressive und ängstliche Verhaltensstrukturen sowie die kognitive Leistungsfähigkeit beurteilen zu können. Nach Ablauf des Beobachtungszeitraumes wurden das Herz und das Gehirn entnommen und weiteren histologischen und molekularen Untersuchungen zugeführt.
Die histologische Aufarbeitung des Herzens nach Ende des Versuchszeitraumes bestätigte die Beobachtungen anderen Autoren, dass eine Infarktgröße von mehr als 30% mit sehr hoher Wahrscheinlichkeit zur Entstehung einer Herzinsuffizienz führt.
Im der histologischen Aufarbeitung des Gehirns zeigen sich keine strukturellen Veränderungen bei herzkranken Mäusen, die die beobachteten Änderungen im Verhalten begründen könnten. Insbesondere kann eine hypoxische Hirnschädigung durch eine etwaige Minderperfusion empfindlicher Hirnareale ausgeschlossen werden.
Mäuse, die nach Induktion eines Myokardinfarktes eine Herzinsuffizienz entwickeln, zeigen nach 8 Wochen Depressions-assoziiertes, adynamisches Verhalten sowie eine Verminderung der kognitiven Leistungsfähigkeit, nicht aber Anzeichen von Angststörungen. Diesen Verhaltensänderungen kann kein strukturelles Korrelat im Gehirn zugewiesen werden. Dies ist ein Indiz dafür, dass sich Veränderung auf molekularer Ebene vollziehen, welche sich dem Mikroskop entziehen. Die im Myokard beobachtete Regulation des Serotoninstoffwechsels ist ein möglicher Erklärungsansatz hierfür.
Background: Homoarginine is an amino acid derivative mainly synthesized in the kidney. It is suggested to increase nitric oxide availability, enhance endothelial function and to protect against cardiovascular diseases. We aimed to investigate the relation between homoarginine, kidney function and progression of chronic kidney disease (CKD).
Methods: We measured plasma homoarginine concentrations in baseline samples of the Mild to Moderate Kidney Disease (MMKD) Study, a prospective cohort study of 227 patients with CKD in Europe. Homoarginine concentrations were available in 182 of the baseline samples and in 139 of the prospectively-followed patients. We correlated homoarginine concentrations to parameters of kidney function. The association between homoarginine and progression of CKD was assessed during a follow-up of up to seven years (median 4.45 years, interquartile range 2.54-5.19) using Cox regression analysis. Progression of CKD was defined as doubling of baseline serum creatinine and/or end-stage renal disease.
Results: Study participants were at baseline on average 47 \(\pm\)13 years old and 65% were male. Mean \(\pm\) standard deviation of homoarginine concentrations were \(2.5 \pm 1.1 \mu mol/L\) and concentrations were incrementally lower at lower levels of GFR with mean concentrations of \(2.90 \pm 1.02 \mu mol/L\) (GFR. 90 ml/min), \(2.64 \pm 1.06 \mu mol/L\) (GFR 60-90 ml/min), \(2.52 \pm 1.24 \mu mol/L\) (GFR 30-60 ml/min) and \(2.05 \pm 0.78 \mu mol/L\) (GFR, 30 ml/min), respectively (p = 0.002). The age-and sex-adjusted risk to reach the renal endpoint was significantly higher by 62% with each decrease by one standard deviation (\(1.1 \mu mol/L\)) of homoarginine (HR 1.62, 95% CI 1.16-2.27, p = 0.005). This association was independent of proteinuria (HR 1.56, 95% CI 1.11-2.20, p = 0.01), and was slightly attenuated when adjusting for GFR (HR 1.40 (95% CI 0.98-1.98, p = 0.06).
Conclusions: Homoarginine concentrations are directly correlated with kidney function and are significantly associated with the progression of CKD. Low homoarginine concentrations might be an early indicator of kidney failure and a potential target for the prevention of disease progression which needs further investigations.
The prevalence of chronic heart failure is still increasing making it a major health issue in the 21st century. Tremendous evidence has emerged over the past decades that heart failure is associated with a wide array of mechanisms subsumed under the term "inflammation". Based on the great success of immuno-suppressive treatments in auto-immunity and transplantation, clinical trials were launched targeting inflammatory mediators in patients with chronic heart failure. However, they widely lacked positive outcomes. The failure of the initial study program directed against tumor necrosis factor-a led to the search for alternative therapeutic targets involving a broader spectrum of mechanisms besides cytokines. We here provide an overview of the current knowledge on immune activation in chronic heart failure of different etiologies, summarize clinical studies in the field, address unresolved key questions, and highlight some promising novel therapeutic targets for clinical trials from a translational basic science and clinical perspective.
Background
Guideline-directed medical therapy (GDMT) is the cornerstone in the treatment of patients with heart failure and reduced ejection fraction (HFrEF) and novel substances such as sacubitril/valsartan (S/V) and sodium-glucose co-transporter-2 inhibitors (SGLT2i) have demonstrated marked clinical benefits. We investigated their implementation into real-world HF care in Germany before, during, and after the COVID-19 pandemic period.
Methods
The IQVIA LRx data set is based on ∼80% of 73 million people covered by the German statutory health insurance. Prescriptions of S/V were used as a proxy for HFrEF. Time trends were analysed between Q1/2016 and Q2/2023 for prescriptions for S/V alone and in combination therapy with SGLT2i.
Findings
The number of patients treated with S/V increased from 5260 in Q1/2016 to 351,262 in Q2/2023. The share of patients with combination therapy grew from 0.6% (29 of 5260) to 14.2% (31,128 of 219,762) in Q2/2021, and then showed a steep surge up to 54.8% (192,429 of 351,262) in Q2/2023, coinciding with the release of the European Society of Cardiology (ESC) guidelines for HF in Q3/2021. Women and patients aged >80 years were treated less often with combined therapy than men and younger patients. With the start of the COVID-19 pandemic, the number of patients with new S/V prescriptions dropped by 17.5% within one quarter, i.e., from 26,855 in Q1/2020 to 22,145 in Q2/2020, and returned to pre-pandemic levels only in Q1/2021.
Interpretation
The COVID-19 pandemic was associated with a 12-month deceleration of S/V uptake in Germany. Following the release of the ESC HF guidelines, the combined prescription of S/V and SGLT2i was readily adopted. Further efforts are needed to fully implement GDMT and strengthen the resilience of healthcare systems during public health crises.

Die Vasodilatation von Blutgefäßen wird maßgeblich durch beta1-adrenerge Rezeptoren vermittelt. Die Deletion von alpha2-adrenergen Rezeptoren (alpha2-KO) führt zur Entwicklung einer Herzinsuffizienz nach Aortenstenose. Der Mortalitätanstieg ist mit erhöhten Plasmanoradrenalin-Spiegeln (a2A-KO), bzw. Plasmaadrenalin-Spiegeln (a2C-KO) assoziiert.
Krankheitsbezogenes Interesse und Schulungsbedürfnis bei Patienten mit chronischer Herzinsuffizienz

Thematik: Patientenschulungen haben sich für diverse Erkrankungen als effektiv und effizient erwiesen. Die individuellen Schulungsbedürfnisse von Patienten mit chronischer Herzinsuffizienz wurden bisher kaum untersucht. Methode: In Kooperation des Instituts für Psychotherapie und Medizinische Psychologie mit der Medizinischen Poliklinik und der Medizinischen Klinik der Universität Würzburg wurden 60 Patienten sowie 23 Angehörige anhand eines standardisierten Interviews bezüglich Ihrer Schulungsbedürfnisse befragt. Das Bedürfnis nach mehr Mitsprache bei der Behandlung wurde über Fallvignetten erfasst. Ergebnisse: Es besteht bei 51 % der Patienten ein klares Interesse, an einer Patientenschulung teilzunehmen, wobei das Interesse negativ mit dem Alter korreliert (p=0,00). Bei vorgegebenen Schulungsinhalten zeigt mehr als die Hälfte der Patienten Interesse für medizinische Themen wie Risikofaktoren, Funktion des Herzens sowie Symptomatik. Informationen zu gesunder Ernährung oder Bewegungsprogrammen werden von den Patienten weniger gewünscht, psychosoziale Inhalte wie Krankheits-und Stressbewältigung werden nur von einem Drittel der Patienten gewünscht. Seelisch belastete Patienten interessieren sich mehr für psychosoziale Inhalte. Die Hälfte der befragten Patienten wünscht sich ein partnerschaftliches Arzt-Patient-Verhältnis. Diese Patienten sind tendenziell jünger (p=0,01) und zeigen größeres Schulungsinteresse (p=0,00). Die Angehörigen der Patienten zeigen mit 61 % großes Schulungsinteresse. Das Interessensprofil ist jedoch anders als bei den Patienten. Schlussfolgerung: Im Rahmen einer Patientenschulung sollten in Bezug auf Alter, Inhalte und Didaktik verschiedene Gruppen gebildet werden. Die Zielvorstellungen Empowerment und Shared-Decision-Making werden nicht von allen Patienten getragen. Ein Curriculum auch für Angehörige wird gewünscht.
Die Pathophysiologie der chronischen Herzinsuffizienz nach Myokardinfarkt ist bestimmt durch neurohumorale Aktivierung und durch Umbauprozesse sowohl in der Infarktregion als auch im überlebenden Myokard, dem sog. ventrikulären Remodeling. Dieses ist charakterisiert durch Myokardhypertrophie, reaktive Fibrose und linksventrikuläre Dysfunktion und Dilatation. Welche Rolle hierbei einer verminderten Bioverfügbarkeit von Stickstoffmonoxid (NO) und einer veränderten Expression der NO-Synthasen (NOS) zukommt, ist Gegenstand intensiver Forschung. Man unterscheidet drei Isoformen der NO-Synthasen, wobei der eNOS im kardiovaskulären System vermutlich die größte Bedeutung zukommt. Das neu entwickelte Medikament AVE9488 steigert die eNOS-Transkription. Die vorliegende Arbeit sollte folgende Fragen beantworten: Kann durch die Behandlung mit AVE9488 eine Verbesserung von linksventrikulärem Remodeling und Dysfunktion nach Myokardinfarkt bei Ratten erzielt werden, und wie verändern sich eNOS-Expression und -Aktivität? Männliche Wistar-Ratten wurden ab dem siebten Tag nach Koronarligatur oder Scheinoperation mit AVE9488 oder Placebo behandelt. Nach neun Wochen wurden hämodynamische Messungen durchgeführt und anschließend das Herz entnommen. Nach Myokardinfarkt fanden sich Zeichen einer ausgeprägten linksventrikulären Dysfunktion und charakteristische Veränderungen der linksventrikulären Morphologie im Sinne einer Hypertrophie und Fibrose. Darüber hinaus zeigte sich eine erhöhte Genexpression von ANP und BNP als Marker einer Herzinsuffizienz. Signifikant reduziert war hingegen die Expression der phosphorylierten und damit aktivierten Form der eNOS nach Myokardinfarkt, ebenso wurde eine verminderte NOS-Aktivität gemessen. Im Vergleich zur Placebo-Gruppe verbesserte die Behandlung mit AVE9488 zahlreiche Parameter der linksventrikulären Funktion, reduzierte das linksventrikuläre endsystolische ebenso wie das enddiastolische Volumen und verringerte die rechtsventrikuläre Hypertrophie. Des Weiteren fanden sich eine reduzierte ANP- und BNP-Expression sowie eine verminderte Kollagenmenge im linken Ventrikel. Insgesamt ließ sich somit eine deutliche Verbesserung von linksventrikulärem Remodeling und Dysfunktion feststellen. Die Behandlung mit AVE9488 erhöhte die Gen- und Proteinexpression der eNOS. Zudem besserte sie die nach Myokardinfarkt reduzierte eNOS-Phosphorylierung und die NOS-Aktivität. Die Ergebnisse deuten darauf hin, dass AVE9488 durch Beeinflussung von Expression, Phosphorylierung und Aktivität der eNOS positive Effekte am erkrankten Herzen nach Myokardinfarkt hervorrufen kann. Dies steht im Einklang mit der in zahlreichen Arbeiten beschriebenen kardioprotektiven Wirkung von eNOS-generiertem NO.
Purpose of Review
We review therapeutic approaches aimed at restoring function of the failing heart by targeting mitochondrial reactive oxygen species (ROS), ion handling, and substrate utilization for adenosine triphosphate (ATP) production.
Recent Findings
Mitochondria-targeted therapies have been tested in animal models of and humans with heart failure (HF). Cardiac benefits of sodium/glucose cotransporter 2 inhibitors might be partly explained by their effects on ion handling and metabolism of cardiac myocytes.
Summary
The large energy requirements of the heart are met by oxidative phosphorylation in mitochondria, which is tightly regulated by the turnover of ATP that fuels cardiac contraction and relaxation. In heart failure (HF), this mechano-energetic coupling is disrupted, leading to bioenergetic mismatch and production of ROS that drive the progression of cardiac dysfunction. Furthermore, HF is accompanied by changes in substrate uptake and oxidation that are considered detrimental for mitochondrial oxidative metabolism and negatively affect cardiac efficiency. Mitochondria lie at the crossroads of metabolic and energetic dysfunction in HF and represent ideal therapeutic targets.
Although heart failure following myocardial infarction (MI) represents a major health burden, underlying microstructural and functional changes remain incompletely understood. Here, we report on a case of unexpected MI after treatment with the catecholamine isoproterenol in an experimental imaging study in mice using different state-of-the-art imaging modalities. The decline in cardiac function was documented by ultrahigh-frequency echocardiography and speckle-tracking analyses. Myocardial microstructure was studied ex vivo at a spatial resolution of 100 × 100 × 100 μm\(^{3}\) using diffusion tensor magnetic resonance imaging (DT-MRI) and histopathologic analyses. Two weeks after ISO treatment, the animal showed an apical aneurysm accompanied by reduced radial strain in corresponding segments and impaired global systolic function. DT-MRI revealed a loss of contractile fiber tracts together with a disarray of remaining fibers as corresponding microstructural correlates. This preclinical case report provides valuable insights into pathophysiology and morphologic–functional relations of heart failure following MI using emerging imaging technologies.
Despite important advances in diagnosis and treatment, heart failure (HF) remains a syndrome with substantial morbidity and dismal prognosis. Although implementation and optimization of existing technologies and drugs may lead to better management of HF, new or alternative strategies are desirable. In this regard, basic science is expected to give fundamental inputs, by expanding the knowledge of the pathways underlying HF development and progression, identifying approaches that may improve HF detection and prognostic stratification, and finding novel treatments. Here, we discuss recent basic science insights that encompass major areas of translational research in HF and have high potential clinical impact.
Despite recent therapeutic advances the prognosis of heart failure remains poor. Recent research suggests that heart failure is a heterogeneous syndrome and that many patients have stimulating auto-antibodies directed against the second extracellular loop of the \(β_1\) adrenergic receptor \((β_1EC2)\). In a human-analogous rat model such antibodies cause myocyte damage and heart failure. Here we used this model to test a novel antibody-directed strategy aiming to prevent and/or treat antibody-induced cardiomyopathy. To generate heart failure, we immunised n = 76/114 rats with a fusion protein containing the human β1EC2 (amino-acids 195–225) every 4 weeks; n = 38/114 rats were control-injected with 0.9% NaCl. Intravenous application of a novel cyclic peptide mimicking \(β_1EC2\) (\(β_1EC2-CP\), 1.0 mg/kg every 4 weeks) or administration of the \(β_1-blocker\) bisoprolol (15 mg/kg/day orally) was initiated either 6 weeks (cardiac function still normal, prevention-study, n = 24 (16 treated vs. 8 untreated)) or 8.5 months after the 1st immunisation (onset of cardiomyopathy, therapy-study, n = 52 (40 treated vs. 12 untreated)); n = 8/52 rats from the therapy-study received \(β_1EC2-CP/bisoprolol\) co-treatment. We found that \(β_1EC2-CP\) prevented and (alone or as add-on drug) treated antibody-induced cardiac damage in the rat, and that its efficacy was superior to mono-treatment with bisoprolol, a standard drug in heart failure. While bisoprolol mono-therapy was able to stop disease-progression, \(β_1EC2-CP\) mono-therapy -or as an add-on to bisoprolol- almost fully reversed antibody-induced cardiac damage. The cyclo¬peptide acted both by scavenging free \(anti-β_1EC2-antibodies\) and by targeting \(β_1EC2\)-specific memory B-cells involved in antibody-production. Our model provides the basis for the clinical translation of a novel double-acting therapeutic strategy that scavenges harmful \(anti-β_1EC2-antibodies\) and also selectively depletes memory B-cells involved in the production of such antibodies. Treatment with immuno-modulating cyclopeptides alone or as an add-on to \(β_1\)-blockade represents a promising new therapeutic option in immune-mediated heart failure.
Background
The importance of chronic kidney disease (CKD) and anaemia has not been comprehensively studied in asymptomatic patients at risk for heart failure (HF) versus those with symptomatic HF. We analysed the prevalence, characteristics and prognostic impact of both conditions across American College of Cardiology/American Heart Association (ACC/AHA) precursor and HF stages A–D.
Methods and results
2496 participants from three non-pharmacological German Competence Network HF studies were categorized by ACC/AHA stage; stage C patients were subdivided into C1 and C2 (corresponding to NYHA classes I/II and III, respectively). Overall, patient distribution was 8.1%/35.3%/32.9% and 23.7% in ACC/AHA stages A/B/C1 and C2/D, respectively. These subgroups were stratified by the absence ( – ) or presence ( +) of CKD (estimated glomerular filtration rate [eGFR] < 60 mL/min/1.73m2) and anaemia (haemoglobin in women/men < 12/ < 13 g/dL). The primary outcome was all-cause mortality at 5-year follow-up. Prevalence increased across stages A/B/C1 and C2/D (CKD: 22.3%/23.6%/31.6%/54.7%; anaemia: 3.0%/7.9%/21.7%/33.2%, respectively), with concordant decreases in median eGFR and haemoglobin (all p < 0.001). Across all stages, hazard ratios [95% confidence intervals] for all-cause mortality were 2.1 [1.8–2.6] for CKD + , 1.7 [1.4–2.0] for anaemia, and 3.6 [2.9–4.6] for CKD + /anaemia + (all p < 0.001). Population attributable fractions (PAFs) for 5-year mortality related to CKD and/or anaemia were similar across stages A/B, C1 and C2/D (up to 33.4%, 30.8% and 34.7%, respectively).
Conclusions
Prevalence and severity of CKD and anaemia increased across ACC/AHA stages. Both conditions were individually and additively associated with increased 5-year mortality risk, with similar PAFs in asymptomatic patients and those with symptomatic HF.

Aims
Heart failure (HF) leads to repeat hospitalisations and reduces the duration and quality of life. Pulmonary artery pressure (PAP)‐guided HF management using the CardioMEMS™ HF system was shown to be safe and reduce HF hospitalisation (HFH) rates in New York Heart Association (NYHA) class III patients. However, these findings have not been replicated in health systems outside the United States. Therefore, the CardioMEMS European Monitoring Study for Heart Failure (MEMS‐HF) evaluated the safety, feasibility, and performance of this device in Germany, The Netherlands, and Ireland.
Methods and results
A total of 234 NYHA class III patients (68 ± 11 years, 22% female, ≥1 HFH in the preceding year) from 31 centres were implanted with a CardioMEMS sensor and underwent PAP‐guided HF management. One‐year rates of freedom from device‐ or system‐related complications and from sensor failure (co‐primary outcomes) were 98.3% [95% confidence interval (CI) 95.8–100.0] and 99.6% (95% CI 97.6–100.0), respectively. Survival rate was 86.2%. For the 12 months post‐ vs. pre‐implant, HFHs decreased by 62% (0.60 vs. 1.55 events/patient‐year; hazard ratio 0.38, 95% CI 0.31–0.48; P < 0.0001). After 12 months, mean PAP decreased by 5.1 ± 7.4 mmHg, Kansas City Cardiomyopathy Questionnaire (KCCQ) overall/clinical summary scores increased from 47.0 ± 24.0/51.2 ± 24.8 to 60.5 ± 24.3/62.4 ± 24.1 (P < 0.0001), and the 9‐item Patient Health Questionnaire sum score improved from 8.7 ± 5.9 to 6.3 ± 5.1 (P < 0.0001).
Conclusion
Haemodynamic‐guided HF management proved feasible and safe in the health systems of Germany, The Netherlands, and Ireland. Physician‐directed treatment modifications based on remotely obtained PAP values were associated with fewer HFH, sustainable PAP decreases, marked KCCQ improvements, and remission of depressive symptoms.
Background
Remote monitoring of patients with New York Heart Association (NYHA) functional class III heart failure (HF) using daily transmission of pulmonary artery (PA) pressure values has shown a reduction in HF-related hospitalizations and improved quality of life in patients.
Objectives
PASSPORT-HF is a prospective, randomized, open, multicenter trial evaluating the effects of a hemodynamic-guided, HF nurse-led care approach using the CardioMEMS™ HF-System on clinical end points.
Methods and results
The PASSPORT-HF trial has been commissioned by the German Federal Joint Committee (G-BA) to ascertain the efficacy of PA pressure-guided remote care in the German health-care system. PASSPORT-HF includes adult HF patients in NYHA functional class III, who experienced an HF-related hospitalization within the last 12 months. Patients with reduced ejection fraction must be on stable guideline-directed pharmacotherapy. Patients will be randomized centrally 1:1 to implantation of a CardioMEMS™ sensor or control. All patients will receive post-discharge support facilitated by trained HF nurses providing structured telephone-based care. The trial will enroll 554 patients at about 50 study sites. The primary end point is a composite of the number of unplanned HF-related rehospitalizations or all-cause death after 12 months of follow-up, and all events will be adjudicated centrally. Secondary end points include device/system-related complications, components of the primary end point, days alive and out of hospital, disease-specific and generic health-related quality of life including their sub-scales, and laboratory parameters of organ damage and disease progression.
Conclusions
PASSPORT-HF will define the efficacy of implementing hemodynamic monitoring as a novel disease management tool in routine outpatient care.

Heart failure is one of the growing causes of death especially in developed countries due to longer life expectancy. Although many pharmacological and instrumental therapeutic approaches have been introduced for prevention and treatment of heart failure, there are still limitations and challenges. Nuclear cardiology has experienced rapid growth in the last few decades, in particular the application of single photon emission computed tomography (SPECT) and positron emission tomography (PET), which allow non-invasive functional assessment of cardiac condition including neurohormonal systems involved in heart failure; its application has dramatically improved the capacity for fundamental research and clinical diagnosis. In this article, we review the current status of applying radionuclide technology in non-invasive imaging of neurohormonal system in the heart, especially focusing on the tracers that are currently available. A short discussion about disadvantages and perspectives is also included.
Aims
Despite recent advances in the treatment of chronic heart failure (HF), mortality and hospitalizations still remain high. Additional therapies to improve mortality and morbidity are urgently needed. The efficacy of cardiac glycosides – although regularly used for HF treatment – remains unclear. DIGIT-HF was designed to demonstrate that digitoxin on top of standard of care treatment improves mortality and morbidity in patients with HF and a reduced ejection fraction (HFrEF).
Methods
Patients with chronic HF, New York Heart Association (NYHA) functional class III–IV and left ventricular ejection fraction (LVEF) ≤ 40%, or patients in NYHA functional class II and LVEF ≤ 30% are randomized 1:1 in a double-blind fashion to treatment with digitoxin (target serum concentration 8–18 ng/mL) or matching placebo. Randomization is stratified by centre, sex, NYHA functional class (II, III, or IV), atrial fibrillation, and treatment with cardiac glycosides at baseline. A total of 2190 eligible patients will be included in this clinical trial (1095 per group). All patients receive standard of care treatment recommended by expert guidelines upon discretion of the treating physician. The primary outcome is a composite of all-cause mortality or hospital admission for worsening HF (whatever occurs first). Key secondary endpoints are all-cause mortality, hospital admission for worsening HF, and recurrent hospital admission for worsening HF.
Conclusion
The DIGIT-HF trial will provide important evidence, whether the cardiac glycoside digitoxin reduces the risk for all-cause mortality and/or hospital admission for worsening HF in patients with advanced chronic HFrEF on top of standard of care treatment.
Background: International disease management guidelines recommend the regular assessment of depression and anxiety in heart failure patients. Currently there is little data on the effect of screening for depression and anxiety on the quality of life and the prognosis of heart failure (HF). We will investigate the association between the recognition of current depression/anxiety by the general practitioner (GP) and the quality of life and the patients' prognosis.
Methods/Design: In this multicenter, prospective, observational study 3,950 patients with HF are recruited by general practices in Germany. The patients fill out questionnaires at baseline and 12-month follow-up. At baseline the GPs are interviewed regarding the somatic and psychological comorbidities of their patients. During the follow-up assessment, data on hospitalization and mortality are provided by the general practice. Based on baseline data, the patients are allocated into three observation groups: HF patients with depression and/or anxiety recognized by their GP (P+/+), those with depression and/or anxiety not recognized (P+/-) and patients without depression and/or anxiety (P-/-). We will perform multivariate regression models to investigate the influence of the recognition of depression and/or anxiety on quality of life at 12 month follow-up, as well as its influences on the prognosis (hospital admission, mortality).
Discussion: We will display the frequency of GP-acknowledged depression and anxiety and the frequency of installed therapeutic strategies. We will also describe the frequency of depression and anxiety missed by the GP and the resulting treatment gap. Effects of correctly acknowledged and missed depression/anxiety on outcome, also in comparison to the outcome of subjects without depression/anxiety will be addressed. In case results suggest a treatment gap of depression/anxiety in patients with HF, the results of this study will provide methodological advice for the efficient planning of further interventional research.
Das aktivierte Komplementsystem, als Teil der angeborenen Immunantwort nach Myokardinfarkt, beeinflusst entscheidend das kardiale Remodeling. Mäuse, die für den Komplementfaktor C3 defizient waren, wiesen acht Wochen nach Infarkt eine signifikant geringere linksventrikuläre Dilatation auf. Anhand von MBL-KO Mäusen sollte in dieser Arbeit die Frage geklärt werden, inwieweit die Aktivierung des Komplementsystems im kardialen Remodeling auf den durch MBL eingeleiteten Pfad zurückgeht. Während sich bezüglich der Infarktgrößen, der Neutrophilen und des Kollagengehalts kein signifikanter Unterschied zwischen den beiden Gruppen zeigte, so wiesen die MBL-KO Tiere im Vergleich zu den WT Tieren eine signifikant größere ventrikuläre Dilatation auf. Basierend auf diesen Erkenntnissen kommen wir zu dem Schluss, dass sich die bezüglich der Ventrikelgröße positiven Effekte einer C3 Hemmung nicht mit einer MBL Hemmung in Einklang bringen lassen. Die dauerhafte Aktivierung des Komplementsystems während des ventrikulären Remodelings, beruht angesichts der Aggravierung der linksventrikulären Dilatation nicht auf dem MBL-Weg.
Aims
Inhibition of neprilysin and angiotensin II receptor by sacubitril/valsartan (Val) (LCZ696) reduces mortality in heart failure (HF) patients compared with sole inhibition of renin–angiotensin system. Beneficial effects of increased natriuretic peptide levels upon neprilysin inhibition have been proposed, whereas direct effects of sacubitrilat (Sac) (LBQ657) on myocardial Ca\(^{2+}\) cycling remain elusive.
Methods and results
Confocal microscopy (Fluo‐4 AM) was used to investigate pro‐arrhythmogenic sarcoplasmic reticulum (SR) Ca\(^{2+}\) leak in freshly isolated murine and human ventricular cardiomyocytes (CMs) upon Sac (40 μmol/L)/Val (13 μmol/L) treatment. The concentrations of Sac and Val equalled plasma concentrations of LCZ696 treatment used in PARADIGM‐HF trial. Epifluorescence microscopy measurements (Fura‐2 AM) were performed to investigate effects on systolic Ca\(^{2+}\) release, SR Ca\(^{2+}\) load, and Ca\(^{2+}\)‐transient kinetics in freshly isolated murine ventricular CMs. The impact of Sac on myocardial contractility was evaluated using in toto‐isolated, isometrically twitching ventricular trabeculae from human hearts with end‐stage HF. Under basal conditions, the combination of Sac/Val did not influence diastolic Ca\(^{2+}\)‐spark frequency (CaSpF) nor pro‐arrhythmogenic SR Ca\(^{2}\) leak in isolated murine ventricular CMs (n CMs/hearts = 80/7 vs. 100/7, P = 0.91/0.99). In contrast, Sac/Val treatment reduced CaSpF by 35 ± 9% and SR Ca\(^{2+}\) leak by 45 ± 9% in CMs put under catecholaminergic stress (isoproterenol 30 nmol/L, n = 81/7 vs. 62/7, P < 0.001 each). This could be attributed to Sac, as sole Sac treatment also reduced both parameters by similar degrees (reduction of CaSpF by 57 ± 7% and SR Ca2+ leak by 76 ± 5%; n = 101/4 vs. 108/4, P < 0.01 each), whereas sole Val treatment did not. Systolic Ca2+ release, SR Ca\(^{2+}\) load, and Ca\(^{2+}\)‐transient kinetics including SERCA activity (k\(_{SERCA}\)) were not compromised by Sac in isolated murine CMs (n = 41/6 vs. 39/6). Importantly, the combination of Sac/Val and Sac alone also reduced diastolic CaSpF and SR Ca\(^{2+}\) leak (reduction by 74 ± 7%) in human left ventricular CMs from patients with end‐stage HF (n = 71/8 vs. 78/8, P < 0.05 each). Myocardial contractility of human ventricular trabeculae was not acutely affected by Sac treatment as the developed force remained unchanged over a time course of 30 min (n trabeculae/hearts = 3/3 vs. 4/3).
Conclusion
This study demonstrates that neprilysin inhibitor Sac directly improves Ca\(^{2+}\) homeostasis in human end‐stage HF by reducing pro‐arrhythmogenic SR Ca\(^{2+}\) leak without acutely affecting systolic Ca\(^{2+}\) release and inotropy. These effects might contribute to the mortality benefits observed in the PARADIGM‐HF trial.
Safety and tolerability of SGLT2 inhibitors in cardiac amyloidosis — a clinical feasibility study

Sodium-glucose transport protein 2 inhibitors (SGLT2i) slow the progression of renal dysfunction and improve the prognosis of patients with heart failure. Amyloidosis constitutes an important subgroup for which evidence is lacking. Amyloidotic fibrils originating from misfolded transthyretin and light chains are the causal agents in ATTR and AL amyloidosis. In these most frequent subtypes, cardiac involvement is the most common organ manifestation. Because cardiac and renal function frequently deteriorate over time, even under best available treatment, SGLT2i emerge as a promising treatment option due to their reno- and cardioprotective properties. We retrospectively analyzed patients with cardiac amyloidosis, who received either dapagliflozin or empagliflozin. Out of 79 patients, 5.1% had urinary tract infections; 2 stopped SGLT2i therapy; and 2.5% died unrelated to the intake of SGLT2i. No genital mycotic infections were observed. As expected, a slight drop in the glomerular filtration rate was noted, while the NYHA functional status, cardiac and hepatic function, as well as the 6 min walk distance remained stable over time. These data provide a rationale for the use of SGLT2i in patients with amyloidosis and concomitant cardiac or renal dysfunction. Prospective randomized data are desired to confirm safety and to prove efficacy in this increasingly important group of patients.
Background
Chronic heart failure (HF) is known to increase the risk of developing Alzheimer’s dementia significantly. Thus, detecting and preventing mild cognitive impairment, which is common in patients with HF, is of great importance. Serum biomarkers are increasingly used in neurological disorders for diagnostics, monitoring, and prognostication of disease course. It remains unclear if neuronal biomarkers may help detect cognitive impairment in this high-risk population. Also, the influence of chronic HF and concomitant renal dysfunction on these biomarkers is not well understood.
Methods
Within the monocentric Cognition.Matters-HF study, we quantified the serum levels of phosphorylated tau protein 181 (pTau) and neurofilament light chain (NfL) of 146 extensively phenotyped chronic heart failure patients (aged 32 to 85 years; 15.1% women) using ultrasensitive bead-based single-molecule immunoassays. The clinical work-up included advanced cognitive testing and cerebral magnetic resonance imaging (MRI).
Results
Serum concentrations of NfL ranged from 5.4 to 215.0 pg/ml (median 26.4 pg/ml) and of pTau from 0.51 to 9.22 pg/ml (median 1.57 pg/ml). We detected mild cognitive impairment (i.e., T-score < 40 in at least one cognitive domain) in 60% of heart failure patients. pTau (p = 0.014), but not NfL, was elevated in this group. Both NfL (ρ = − 0.21; p = 0.013) and pTau (ρ = − 0.25; p = 0.002) related to the cognitive domain visual/verbal memory, as well as white matter hyperintensity volume and cerebral and hippocampal atrophy. In multivariable analysis, both biomarkers were independently influenced by age (T = 4.6 for pTau; T = 5.9 for NfL) and glomerular filtration rate (T = − 2.4 for pTau; T = − 3.4 for NfL). Markers of chronic heart failure, left atrial volume index (T = 4.6) and NT-proBNP (T = 2.8), were further cardiological determinants of pTau and NfL, respectively. In addition, pTau was also strongly affected by serum creatine kinase levels (T = 6.5) and ferritin (T = − 3.1).
Conclusions
pTau and NfL serum levels are strongly influenced by age-dependent renal and cardiac dysfunction. These findings point towards the need for longitudinal examinations and consideration of frequent comorbidities when using neuronal serum biomarkers.
Aims
There is an ongoing discussion whether the categorization of patients with heart failure according to left ventricular ejection fraction (LVEF) is scientifically justified and clinically relevant. Major efforts are directed towards the identification of appropriate cut-off values to correctly allocate heart failure-specific pharmacotherapy. Alternatively, an LVEF continuum without definite subgroups is discussed. This study aimed to evaluate the natural distribution of LVEF in patients presenting with acutely decompensated heart failure and to identify potential subgroups of LVEF in male and female patients.
Methods and results
We identified 470 patients (mean age 75 ± 11 years, n = 137 female) hospitalized for acute heart failure in whom LVEF could be quantified by Simpson's method in an in-hospital echocardiogram. Non-parametric modelling revealed a bimodal shape of the LVEF distribution. Parametric modelling identified two clusters suggesting two LVEF peaks with mean (variance) of 61% (9%) and 31% (10%), respectively. Sub-differentiation by sex revealed a sex-specific bimodal clustering of LVEF. The respective threshold differentiating between ‘high’ and ‘low’ LVEF was 45% in men and 52% in women.
Conclusions
In patients presenting with acute heart failure, LVEF clustered in two subgroups and exhibited profound sex-specific distributional differences. These findings might enrich the scientific process to identify distinct subgroups of heart failure patients, which might each benefit from respectively tailored (pharmaco)therapies.
Risk prediction in patients with heart failure (HF) is essential to improve the tailoring of preventive, diagnostic, and therapeutic strategies for the individual patient, and effectively use health care resources. Risk scores derived from controlled clinical studies can be used to calculate the risk of mortality and HF hospitalizations. However, these scores are poorly implemented into routine care, predominantly because their calculation requires considerable efforts in practice and necessary data often are not available in an interoperable format. In this work, we demonstrate the feasibility of a multi-site solution to derive and calculate two exemplary HF scores from clinical routine data (MAGGIC score with six continuous and eight categorical variables; Barcelona Bio-HF score with five continuous and six categorical variables). Within HiGHmed, a German Medical Informatics Initiative consortium, we implemented an interoperable solution, collecting a harmonized HF-phenotypic core data set (CDS) within the openEHR framework. Our approach minimizes the need for manual data entry by automatically retrieving data from primary systems. We show, across five participating medical centers, that the implemented structures to execute dedicated data queries, followed by harmonized data processing and score calculation, work well in practice. In summary, we demonstrated the feasibility of clinical routine data usage across multiple partner sites to compute HF risk scores. This solution can be extended to a large spectrum of applications in clinical care.
Background: \(^{18}\)F-N-[3-bromo-4-(3-fluoro-propoxy)-benzyl]-guanidine (\(^{18}\)F-LMI1195) is a new class of PET tracer designed for sympathetic nervous imaging of the heart. The favorable image quality with high and specific neural uptake has been previously demonstrated in animals and humans, but intracellular behavior is not yet fully understood. The aim of the present study is to verify whether it is taken up in storage vesicles and released in company with vesicle turnover.
Results: Both vesicle-rich (PC12) and vesicle-poor (SK-N-SH) norepinephrine-expressing cell lines were used for in vitro tracer uptake studies. After 2 h of \(^{18}\)F-LMI1195 preloading into both cell lines, effects of stimulants for storage vesicle turnover (high concentration KCl (100 mM) or reserpine treatment) were measured at 10, 20, and 30 min. \(^{131}\)I-meta-iodobenzylguanidine (\(^{131}\)I-MIBG) served as a reference. Both high concentration KCl and reserpine enhanced \(^{18}\)F-LMI1195 washout from PC12 cells, while tracer retention remained stable in the SK-N-SH cells. After 30 min of treatment, 18F-LMI1195 releasing index (percentage of tracer released from cells) from vesicle-rich PC12 cells achieved significant differences compared to cells without treatment condition. In contrast, such effect could not be observed using vesicle-poor SK-N-SH cell lines. Similar tracer kinetics after KCl or reserpine treatment were also observed using 131I-MIBG. In case of KCl exposure, Ca\(^{2+}\)-free buffer with the calcium chelator, ethylenediaminetetracetic acid (EDTA), could suppress the tracer washout from PC12 cells. This finding is consistent with the tracer release being mediated by Ca\(^{2+}\) influx resulting from membrane depolarization.
Conclusions: Analogous to \(^{131}\)I-MIBG, the current in vitro tracer uptake study confirmed that \(^{131}\)F-LMI1195 is also stored in vesicles in PC12 cells and released along with vesicle turnover. Understanding the basic kinetics of \(^{18}\)FLMI1195 at a subcellular level is important for the design of clinical imaging protocols and imaging interpretation.
Alzheimer's disease (AD), the most common cause of dementia in the elderly, is a neurodegenerative disorder associated with neurovascular dysfunction and cognitive decline. While the deposition of amyloid β peptide (Aβ) and the formation of neurofibrillary tangles (NFTs) are the pathological hallmarks of AD-affected brains, the majority of cases exhibits a combination of comorbidities that ultimately lead to multi-organ failure. Of particular interest, it can be demonstrated that Aβ pathology is present in the hearts of patients with AD, while the formation of NFT in the auditory system can be detected much earlier than the onset of symptoms. Progressive hearing impairment may beget social isolation and accelerate cognitive decline and increase the risk of developing dementia. The current review discusses the concept of a brain–ear–heart axis by which Aβ and NFT inhibition could be achieved through targeted supplementation of neurotrophic factors to the cochlea and the brain. Such amyloid inhibition might also indirectly affect amyloid accumulation in the heart, thus reducing the risk of developing AD-associated amyloid cardiomyopathy and cardiovascular disease.
Diabetes mellitus is a common disease affecting more than 537 million adults worldwide. The microvascular complications that occur during the course of the disease are widespread and affect a variety of organ systems in the body. Diabetic retinopathy is one of the most common long-term complications, which include, amongst others, endothelial dysfunction, and thus, alterations in the blood-retinal barrier (BRB). This particularly restrictive physiological barrier is important for maintaining the neuroretina as a privileged site in the body by controlling the inflow and outflow of fluid, nutrients, metabolic end products, ions, and proteins. In addition, people with diabetic retinopathy (DR) have been shown to be at increased risk for systemic vascular complications, including subclinical and clinical stroke, coronary heart disease, heart failure, and nephropathy. DR is, therefore, considered an independent predictor of heart failure. In the present review, the effects of diabetes on the retina, heart, and kidneys are described. In addition, a putative common microRNA signature in diabetic retinopathy, nephropathy, and heart failure is discussed, which may be used in the future as a biomarker to better monitor disease progression. Finally, the use of miRNA, targeted neurotrophin delivery, and nanoparticles as novel therapeutic strategies is highlighted.
OBJECTIVES: This study evaluated the tolerability and feasibility of titration of 2 distinctly acting beta-blockers (BB) in elderly heart failure patients with preserved (HFpEF) and reduced (HFrEF) left ventricular ejection fraction.
BACKGROUND: Broad evidence supports the use of BB in HFrEF, whereas the evidence for beta blockade in HFpEF is uncertain.
METHODS: In the CIBIS-ELD (Cardiac Insufficiency Bisoprolol Study in Elderly) trial, patients >65 years of age with HFrEF (n = 626) or HFpEF (n = 250) were randomized to bisoprolol or carvedilol. Both BB were up-titrated to the target or maximum tolerated dose. Follow-up was performed after 12 weeks. HFrEF and HFpEF patients were compared regarding tolerability and clinical effects (heart rate, blood pressure, systolic and diastolic functions, New York Heart Association functional class, 6-minute-walk distance, quality of life, and N-terminal pro-B-type natriuretic peptide).
RESULTS: For both of the BBs, tolerability and daily dose at 12 weeks were similar. HFpEF patients demonstrated higher rates of dose escalation delays and treatment-related side effects. Similar HR reductions were observed in both groups (HFpEF: 6.6 beats/min; HFrEF: 6.9 beats/min, p = NS), whereas greater improvement in NYHA functional class was observed in HFrEF (HFpEF: 23% vs. HFrEF: 34%, p < 0.001). Mean E/e' and left atrial volume index did not change in either group, although E/A increased in HFpEF. CONCLUSIONS: BB tolerability was comparable between HFrEF and HFpEF. Relevant reductions of HR and blood pressure occurred in both groups. However, only HFrEF patients experienced considerable improvements in clinical parameters and Left ventricular function. Interestingly, beta-blockade had no effect on established and prognostic markers of diastolic function in either group. Long-term studies using modern diagnostic criteria for HFpEF are urgently needed to establish whether BB therapy exerts significant clinical benefit in HFpEF. Fibrosis is a pivotal player in heart failure development and progression. Measurements of (markers of) fibrosis in tissue and blood may help to diagnose and risk stratify patients with heart failure, and its treatment may be effective in preventing heart failure and its progression. A lack of pathophysiological insights and uniform definitions has hampered the research in fibrosis and heart failure. The Translational Research Committee of the Heart Failure Association discussed several aspects of fibrosis in their workshop. Early insidious perturbations such as subclinical hypertension or inflammation may trigger first fibrotic events, while more dramatic triggers such as myocardial infarction
and myocarditis give rise to full blown scar formation and ongoing fibrosis in diseased hearts. Aging itself is also associated with a cardiac phenotype that includes fibrosis. Fibrosis is an extremely heterogeneous phenomenon, as several stages of the fibrotic process exist, each with different fibrosis subtypes and a different composition of various cells and proteins — resulting in a very complex pathophysiology. As a result, detection of fibrosis, e.g. using current cardiac imaging modalities or plasma biomarkers, will detect only specific subforms of fibrosis, but cannot capture all aspects of the complex fibrotic process. Furthermore, several anti-fibrotic therapies are under investigation, but such therapies generally target aspecific aspects of the fibrotic process and suffer from a lack of precision. This review discusses the mechanisms and the caveats and proposes a roadmap for future research.
Transgene Mausmodelle zur Charakterisierung der Funktion kardialer beta-adrenerger Rezeptoren

In der vorliegenden Arbeit wurde die Funktion kardialer beta-adrenerger Rezeptoren mit Hilfe einer Kombination aus transgenen Mausmodellen und physiologischen und molekularbiologischen Methoden untersucht. Durch gezielte Überexpression des humanen beta1-adrenergen Rezeptors im Herzen transgener Mäuse konnte gezeigt werden, daß die chronische Aktivierung dieses Rezeptors eine trophische Wirkung auf die Herzmuskelzellen hat. Über einen Zeitraum von mehreren Monaten führte dies zur Entwicklung einer Herzinsuffizienz. In der menschlichen Herzinsuffizienz kommt es zu einem ähnlichen Phänomen: Durch deutlich erhöhte Freisetzung von endogenen Katecholaminen kommt es zu einer chronischen Dauerstimulation kardialer beta1-adrenerger Rezeptoren. Daß diese schädlich ist belegen das hier beschriebene Mausmodell und zudem einige neuere klinische Studien, die zeigen daß eine pharmakologische Blockade beta-adrenerger Rezeptoren zu einer Verminderung der Herzinsuffizienzmortalität führt. Dieses Mausmodell erlaubte es erstmals den beta1-adrenergen Rezeptor hinsichtlich seiner spontanen Rezeptoraktivität in einem physiologischen Modell zu untersuchen. Dabei zeigte sich, daß der humane beta1-adrenerge Rezeptor spontane Aktivität aufweist, jedoch in einem deutlich geringeren Ausmaß als der beta2-adrenerge Rezeptor. Dies könnte klinisch relevant sein, da klinisch verwendete beta-Rezeptor-Antagonisten die spontane Aktivität des beta1-adrenergen Rezeptors in unserem Modell unterschiedlich stark unterdrückten. In der vorliegenden Arbeit wurde zudem untersucht, ob sich die beiden kardial exprimierten Beta-Rezeptor-Subtypen Beta1 und Beta2 hinsichtlich ihrer Signaltransduktion unterscheiden. Ausgehend von dem Befund, daß die chronische Aktivierung der beiden Subtypen in transgenen Mausmodellen zu deutlich unterschiedlichen Phänotypen führt, wurden verschiedene intrazelluläre Signalwege auf ihre Aktivierung hin überprüft. Abweichend von publizierten, in vitro nach kurzzeitiger Rezeptorstimulation erhobenen Daten zeigte sich, daß die chronische Aktivierung der Rezeptorsubtypen zu einer unterschiedlichen Aktivierung der kardialen MAP-kinasen (ERK) führt. Die beta1-spezifische Aktivierung dieser Kinasen könnte die beobachtete unterschiedliche Hypertrophieentwicklung in diesen beiden Mausmodellen erklären. Einen weiteren Schwerpunkt bei der Aufklärung des Mechanismus beta-adrenerg induzierter Hypertrophie bildete die Untersuchung der zellulären Calcium-homöostase. Als früheste funktionelle Veränderung in der Entwicklung einer beta-adrenerg induzierten Herzhypertrophie und -insuffizienz trat dabei eine Störung des intrazellulären Calciumtransienten auf. Als möglicher Mechanismus für die Störung des Calciumhaushalts konnte eine zeitgleich auftretende veränderte Expression des Calcium-regulierenden Proteins Junctin beschrieben werden. Einen neuen therapeutischen Ansatz für die Therapie der Herzinsuffizienz könnten schließlich vielleicht die Untersuchungen zum kardialen Na/H-austauscher ergeben: Es konnte erstmals gezeigt werden, daß der kardiale Na/H-Austauscher maßgeblich an der beta-adrenerg induzierten Herzhypertrophie- und Fibrose-entstehung beteiligt ist und daß die pharmakologische Inhibition dieses Proteins sowohl Hypertrophie als auch die Fibrose wirksam unterdrücken kann.
Die Herzinsuffizienz ist eine der führenden Todesursachen weltweit. Eine auf die neurohumorale Aktivierung zugeschnittene Therapie mit ACE-Hemmern bzw. Angiotensin-Rezeptorantagonisten, Betablockern, Aldosteronantagonisten und Diuretika verbessert zwar die Symptomatik und Prognose. Letztere ist bei Diagnosestellung jedoch immer noch schlechter als die vieler maligner Erkrankungen einzuschätzen. Ziel ist daher die Entwicklung von Medikamenten, die den Krankheitsverlauf des Syndroms Herzinsuffizienz aufhalten bzw. umkehren. Ein Ansatz ist dabei die Analyse sogenannter Kandidatengene, die im kranken Herzen differentiell exprimiert werden und potentiell medikamentös beeinflussbar sind. Im Rahmen der vorliegenden Arbeit wurden zwei solcher Kandidatengene charakterisiert. Mäuse mit kardialer Überexpression b1-adrenerger Rezeptoren entwickeln eine Herzinsuffizienz mit kardialer Hypertrophie und Fibrose. In Gen Arrays mit 21000 Maus-ESTs zeigten sich unter anderem die Gene des Uncoupling Protein 2 (UCP2) und der Diphosphoinositol-Polyphosphat-Phosphohydrolase 2 alpha (DIPP2a) aktiviert. Diese Befunde wurden zunächst mittels RNase Protection Assay und RT-PCR bestätigt. Auch andere murine Herzinsuffizienzmodelle wurden untersucht. So ließ sich ebenfalls im druckinduzierten Herzinsuffizienzmodell nach artifizieller Aortenstenose sowie im b2-AR überexprimierenden Herzen eine erhöhte Konzentration von UCP2- und DIPP2a-mRNA messen. Um zu prüfen, ob diese differentielle mRNA-Expression deletäre oder protektive Effekte vermittelt, wurden jeweils transgene Mauslinien mit herzspezifischer Überexpression von UCP2 und DIPP2a generiert. Die Linie UCP2-TG1 mit hoher Überexpression sowie ein Gründer-Tier der UCP2-transgenen Mäuse entwickelten eine Herzinsuffizienz mit vergrößertem Herzen, linksventrikulärem Pumpversagen, interstitieller Fibrose und typischen Veränderungen der molekularen Marker ANF und SERCA. Zudem fanden sich dilatierte Vorhöfe sowie eine bradykarde Herzrhythmusstörung. UCP2 ist ein Entkoppler der oxidativen Phosphorylierung im Mitochondrium. Im energieintensiven Stoffwechsel des Myokards könnte eine durch UCP2 reduzierte ATP-Synthese zu den genannten Veränderungen führen. Für UCP2 wurden auch protektive Eigenschaften durch das Abfangen freier Radikale beschrieben. Die Linie UCP2-TG3 mit niedrigerem Überexpressionsniveau und ohne kardialen Phänotyp wurde deshalb einem Aortic banding unterzogen, wo sich in der Überlebenskurve kein protektiver oder deletärer Effekt einer moderat vermehrten Entkopplung im Vergleich zum Wildtyp zeigte. In drei unabhängigen Linien transgener Mäuse mit herzspezifischer Überexpression von DIPP2a ließ sich morphometrisch eine kardiale Hypertrophie nachweisen. Die Linie DIPP2a-TG9 mit dem höchsten Überexpressionsniveau zeigte zudem eine kardiale Fibrose sowie unter Dobutamin eine verminderte kardiale Kontraktilitätsreserve im Vergleich zum Wildtypen. DIPP-Proteine hydrolysieren Inositolphosphate und Nukleosiddiphosphate und greifen so in zentrale Stoffwechselvorgänge ein, die im einzelnen noch nicht geklärt sind. Es konnte hier im Mausmodell gezeigt werden, dass UCP2 und DIPP2a zwei für die Entwicklung einer Herzinsuffizienz relevante Zielproteine darstellen. Geplant ist die weitere Aufklärung der beteiligten Mechanismen, um diese letztlich auch therapeutisch beeinflussen zu können.
Mutations in mitochondrial aminoacyl-tRNA synthetases (mtARSs) have been reported in patients with mitochondriopathies: most commonly encephalopathy, but also cardiomyopathy. Through a GWAS, we showed possible associations between mitochondrial valyl-tRNA synthetase (VARS2) dysregulations and non-ischemic cardiomyopathy. We aimed to investigate the possible consequences of VARS2 depletion in zebrafish and cultured HEK293A cells. Transient VARS2 loss-of-function was induced in zebrafish embryos using Morpholinos. The enzymatic activity of VARS2 was measured in VARS2-depleted cells via northern blot. Heterozygous VARS2 knockout was established in HEK293A cells using CRISPR/Cas9 technology. BN-PAGE and SDS-PAGE were used to investigate electron transport chain (ETC) complexes, and the oxygen consumption rate and extracellular acidification rate were measured using a Seahorse XFe96 Analyzer. The activation of the integrated stress response (ISR) and possible disruptions in mitochondrial fatty acid oxidation (FAO) were explored using RT-qPCR and western blot. Zebrafish embryos with transient VARS2 loss-of-function showed features of heart failure as well as indications of CNS and skeletal muscle involvements. The enzymatic activity of VARS2 was significantly reduced in VARS2-depleted cells. Heterozygous VARS2-knockout cells showed a rearrangement of ETC complexes in favor of complexes III\(_2\), III\(_2\) + IV, and supercomplexes without significant respiratory chain deficiencies. These cells also showed the enhanced activation of the ISR, as indicated by increased eIF-2α phosphorylation and a significant increase in the transcript levels of ATF4, ATF5, and DDIT3 (CHOP), as well as disruptions in FAO. The activation of the ISR and disruptions in mitochondrial FAO may underlie the adaptive changes in VARS2-depleted cells.
Herzinsuffizienz ist eine sehr häufige Erkrankung vor allem des höheren Lebensalters. Biomarker wie NT-proBNP, BNP, hsCRP haben neben ihrer Bedeutung für die Diagnose einer akuten Herzinsuffizienz einen großen Stellenwert in der Abschätzung der Prognose eines Patienten. Die prognostische Relevanz dieser Marker konnte auch bei nicht herzinsuffizienten, anderweitig kranken Patienten gezeigt werden. Unklar und wenig erforscht ist die Aussagekraft von Biomarkern in einem Kollektiv nicht akut dekompensierter Patienten, welche sich ambulant bei ihrem Hausarzt vorstellen. Die Handheld-BNP-Studie untersuchte im primärärztlichen Bereich das diagnostische Potential von BNP und der miniaturisierten Echokardiographie. Die vorliegende Follow-up-Studie untersucht die prognostische Relevanz von BNP sowie vergleichend den prognostischen Wert von NT-proBNP und der Kardiologendiagnose. Auch die prognostische Aussagekraft der inflammatorischen Marker hsCRP und TNF-ɑ, ebenso wie die Frage, ob durch eine Kombination der Marker die prognostische Abschätzung weiter gesteigerter werden kann, ist Gegenstand dieser Arbeit. Zuletzt wurde eine multivariate Regressionsanalyse durchgeführt, um den unabhängigen prognostischen Wert der Biomarker zu untersuchen. Es konnte gezeigt werden, dass bei diagnostisch naiven Patienten mit dem klinisch-anamnestischen Verdacht auf das Vorliegen einer Herzinsuffizienz das kardiale wie auch das nicht-kardiale Mortalitätsrisiko sowie die Rate an Hospitalisierungen gegenüber der Allgemeinbevölkerung gleichen Alters erhöht sind, unabhängig vom Vorliegen einer Herzinsuffizienz. Eine Bestimmung der Biomarker BNP, NT-proBNP, hsCRP und TNF-ɑ erwies sich in diesem Kollektiv als hilfreich, diejenigen mit erhöhtem Risiko zu erkennen.
Aims
Vitamin D deficiency is prevalent in heart failure (HF), but its relevance in early stages of heart failure with preserved ejection fraction (HFpEF) is unknown. We tested the association of 25-hydroxyvitamin D [25(OH)D] serum levels with mortality, hospitalizations, cardiovascular risk factors, and echocardiographic parameters in patients with asymptomatic diastolic dysfunction (DD) or newly diagnosed HFpEF.
Methods and results
We measured 25(OH)D serum levels in outpatients with risk factors for DD or history of HF derived from the DIAST-CHF study. Participants were comprehensively phenotyped including physical examination, echocardiography, and 6 min walk test and were followed up to 5 years. Quality of life was evaluated by the Short Form 36 (SF-36) questionnaire. We included 787 patients with available 25(OH)D levels. Median 25(OH)D levels were 13.1 ng/mL, mean E/e′ medial was 13.2, and mean left ventricular ejection fraction was 59.1%. Only 9% (n = 73) showed a left ventricular ejection fraction <50%. Fifteen per cent (n = 119) of the recruited participants had symptomatic HFpEF. At baseline, participants with 25(OH)D levels in the lowest tertile (≤10.9 ng/L; n = 263) were older, more often symptomatic (oedema and fatigue, all P ≤ 0.002) and had worse cardiac [higher N-terminal pro-brain natriuretic peptide (NT-proBNP) and left atrial volume index, both P ≤ 0.023], renal (lower glomerular filtration rate, P = 0.012), metabolic (higher uric acid levels, P < 0.001), and functional (reduced exercise capacity, 6 min walk distance, and SF-36 physical functioning score, all P < 0.001) parameters. Increased NT-proBNP, uric acid, and left atrial volume index and decreased SF-36 physical functioning scores were independently associated with lower 25(OH)D levels. There was a higher risk for lower 25(OH)D levels in association with HF, DD, and atrial fibrillation (all P ≤ 0.004), which remained significant after adjusting for age. Lower 25(OH)D levels (per 10 ng/mL decrease) tended to be associated with higher 5 year mortality, P = 0.05, hazard ratio (HR) 1.55 [1.00; 2.42]. Furthermore, lower 25(OH)D levels (per 10 ng/mL decrease) were related to an increased rate of cardiovascular hospitalizations, P = 0.023, HR = 1.74 [1.08; 2.80], and remained significant after adjusting for age, P = 0.046, HR = 1.63 [1.01; 2.64], baseline NT-proBNP, P = 0.048, HR = 1.62 [1.01; 2.61], and other selected baseline characteristics and co-morbidities, P = 0.043, HR = 3.60 [1.04; 12.43].
Conclusions
Lower 25(OH)D levels were associated with reduced functional capacity in patients with DD or HFpEF and were significantly predictive for an increased rate of cardiovascular hospitalizations, also after adjusting for age, NT-proBNP, and selected baseline characteristics and co-morbidities.
