Diese Dissertation untersucht das Phänomen Stress im Zusammenhang mit der Tako-Tsubo-Kardiomyopathie. Sie ist eine der ersten Arbeiten, die insbesondere die Stressverarbeitung der Betroffenen mit standardisierten Messverfahren analysiert. In einer retrospektiven Querschnittsstudie wurden mit Hilfe von zwei Fragebögen soziodemographische, medizinische und psychologische Aspekte dieser Erkrankung erfasst; insbesondere wurde die Stressverarbeitung der Betroffenen mit Hilfe des standardisierten Fragebogens SVF-120 von G. Erdmann und W. Janke erfasst. Es zeigt sich, dass entgegen unserer Erwartung die Stressverarbeitung von Patienten mit Apical Ballooning Syndrom sich nicht von derjenigen von Gesunden oder Myokardinfarkt-Patienten unterscheidet. Jedoch gaben 80 % unserer befragten Patienten an unter akuten oder längerfristigen Stressoren zum Zeitpunkt des Anfalls gelitten zu haben. Ein Zusammenhang zwischen Stress und der Stressherzerkrankung ist also naheliegend, jedoch scheint eine pathologische Stressverarbeitungsweise nicht der entscheidende Aspekt in der Pathogenese zu sein.
Background
Anastomotic leakage (AL) is one of the most common and serious complications following visceral surgery. In recent years, endoluminal vacuum therapy has dramatically changed therapeutic options for AL, but its use has been limited to areas easily accessible by endoscope.
Case presentation
We describe the first use of endoluminal vacuum therapy in the small intestine employing a combined surgical and endoscopic “rendezvous technique” in which the surgeon assists the endoscopic placement of an endoluminal vacuum therapy sponge in the jejunum by means of a pullback string. This technique led to a completely closed AL after 27 days and 7 changes of the endosponge.
Conclusion
The combined surgical and endoscopic rendezvous technique can be useful in cases of otherwise difficult endosponge placement.
Gastrointestinale Lymphome des MALT sind heute als eigenständige Entität anerkannt. Sie zeichnen sich durch morphologische, molekularbiologische, ätiopathogenetische und biologische Besonderheiten aus, die sie von den nodalen Lymphomen abgrenzen lassen. Im Rahmen der Würzburger Multicenterstudie „Gastrointestinale Lymphome II“ soll die Frage geklärt werden, ob in den lokalisierten Stadien I und II primärer Magenlymphome unter Berücksichtigung der posttherapeutischen Lebensqualität der operativen oder einer primär konservativen Therapie (Chemo- oder Radiotherapie) Vorzug gegeben werden soll. Von 1998 bis 2002 wurden dazu 49 Patienten mit neu diagnostiziertem niedrig- (n=19) oder hochmalignen (n=30) NHL des Magens in die Studie eingeschlossen. Nach zentraler Randomisierung wurden die Patienten mit low grade NHL der Operation (n=10) oder der Radiotherapie (n=9) zugeteilt. Die Patienten mit high grade NHL erhielten Operation plus Chemotherapie (n=16) oder alleinige Chemotherapie (n=14). Das mediane follow up betrug 74 Monate. Sowohl das operative als auch das konservative Vorgehen zeigten bei beiden Lymphomhistologien überaus hohe Remissionsraten in den Stadien EI und EII (CR um 90%). Unterschiede zwischen den Behandlungsmethoden konnten, evtl. auch bedingt durch die kleinen Gruppengrößen, nicht nachgewiesen werden. Insgesamt verstarben sechs Patienten, die sich in etwa gleichmäßig auf die Gruppen verteilten. Die Auswertung der Fragebögen zur Lebensqualität ergab gemäß SF-36 durchaus hohe Werte zur posttherapeutischen Lebensqualität. Eine Differenzierung der Patienten nach Behandlungsmethode oder Malignitätsgrad (low, high grade) war nicht möglich. Der Lebensqualitätsindex nach Troidl ließ einen prä-post-therapeutischen Vergleich zu: Betrachtet man die Kombination von Therapie und Malignitätsgrad, so fallen konservativ therapierte Patienten (unabhängig vom Malignitätsgrad) durch deutlich ansteigende Lebensqualität auf. Es lässt sich festhalten, dass bei primär gastrointestinalen Lymphomen sowohl mit operativem als auch konservativem Vorgehen hohe Remissionsraten erzielt werden können. Jedoch sind im Hinblick auf Organerhalt und die höhere post-therapeutische Lebensqualität die primäre Radio- und/oder Chemotherapie dem operativen Vorgehen vorzuziehen.
Zinc is an essential trace element for all living organisms. In mammals, including humans and mice, it is required for normal growth, development, hematopoiesis and immune defense. This thesis investigates the influence of zinc on the development of megakaryocytes (MKs), the cells responsible for bone marrow-derived platelet production. Furthermore, a detailed analysis of the expression of zinc import and export transporters (Slc39a/Slc30a genes) is carried out, firstly over the course of MK differentiation and secondly dependent on extracellular zinc.
Die Zelladhäsion von Leukozyten/ Metastasenzellen an Endothelzellen bei Entzündungsreaktionen/ Arteriosklerose und Tumormetastasierung ist ein mehrstufiger Prozess. Im ersten Schritt kommt es zu einer Interaktion zwischen E-Selektin-Rezeptoren auf den Endothelzellen und komplexen Kohlenhydraten, den Polylaktosaminen und ihren Derivaten, den Lewis-Antigenen. Im Rahmen dieser Arbeit wurden durch transiente Transfektion fünf rekombinante Glykoproteine erzeugt (hLysII/IV-FucTIII-VII). Durch hLysII/IV-FucTVI konnte die Zelladhäsion von U937 an HUVEC Zellen signifikant gehemmt werden. Fukosyltransferase VI scheint in der Lage zu sein, die Bildung von endständigen sLex-Antigenen an den Polylaktosaminketten von hLys zu ermöglichen. Auch durch eine Transfektion der Kolonkarzinomzelllinie SW480 und Colo 206 konnten Lysozymmutanten hergestellt werden, mit denen eine signifikante Adhäsionblockade möglich ist. Man kann sich hLysII/IV-FucTVI- SW480 und- Colo206 als potentielle therapeutische Substanzen vorstellen, die zum Beispiel zur Entzündungs- oder Metastasierungshemmung eingesetzt werden.
In CHO-FucTVI- Zellen wurde hLysII/IV stabil transfiziert, mit Puromycin selektioniert und hLysII/IV-FucTVI von den stabil transfizierten CHO-FucTVI- Zelle überexprimiert. Durch Immunaffinitätschromatographie und Ultrafiltration wurde das überexprimierte hLysII/IV-FucTVI aufgereinigt und aufkonzentriert. Durch den Lysozymtest nach Osserman und Lawlor und einen ELISA konnte die Lysozymmenge in den unterschiedlichen Schritten bestimmt werden. Im anschließenden Zelladhäsionsassay konnten bei Konzentrationen von 1 ng/ml, 10 ng/ml und 100 ng/ml hLysII/IV-FucTVI signifikante Reduktionen der Zelladhäsion von U937- Zellen an HUVEC- Zellen festgestellt werden. Die ermittelte mittlere Hemmkonzentration (IC50) von hLysII/IV-FucTVI liegt bei 7*10-12 M. Dies entspricht bei einem Molekulargewicht von 30 kDa der Menge von 0,21 ng/ml und hLysII/IV-FucTVI wäre damit der stärkste bisher bekannte E-Selektin-Antagonist. In dieser Funktion könnte hLysII/IV-FucTVI im Rahmen einer antiinflammatischen oder antineoplastischen Therapie eingesetzt werden.
Mutations in the oncogenic PIK3CA gene are found in 10-20% of colorectal cancers (CRCs) and are associated with poor prognosis. Tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) and agonistic TRAIL death receptor antibodies emerged as promising anti-neoplastic therapeutics, but to date failed to prove their capability in the clinical setting as especially primary tumors exhibit high rates of TRAIL resistance. In our study, we investigated the molecular mechanisms underlying TRAIL resistance in CRC cells with a mutant PIK3CA (PIK3CA-mut) gene. We show that inhibition of the constitutively active phosphatidylinositol-3 kinase (PI3K)/Akt signaling pathway only partially overcame TRAIL resistance in PIK3CA-mut-protected HCT116 cells, although synergistic effects of TRAIL plus PI3K, Akt or cyclin-dependent kinase (CDK) inhibitors could be noted. In sharp contrast, TRAIL triggered full-blown cell death induction in HCT116 PIK3CA-mut cells treated with proteasome inhibitors such as bortezomib and MG132. At the molecular level, resistance of HCT116 PIK3CA-mut cells against TRAIL was reflected by impaired caspase-3 activation and we provide evidence for a crucial involvement of the E3-ligase X-linked inhibitor of apoptosis protein (XIAP) therein. Drugs interfering with the activity and/or the expression of XIAP, such as the second mitochondria-derived activator of caspase mimetic BV6 and mithramycin-A, completely restored TRAIL sensitivity in PIK3CA-mut-protected HCT116 cells independent of a functional mitochondrial cell death pathway. Importantly, proteasome inhibitors and XIAP-targeting agents also sensitized other CRC cell lines with mutated PIK3CA for TRAIL-induced cell death. Together, our data suggest that proteasome-or XIAP-targeting drugs offer a novel therapeutic approach to overcome TRAIL resistance in PIK3CA-mutated CRC.
Loss of Somatostatin Receptor 2 (SSTR2) expression and rising CXC Chemokine Receptor Type 4 (CXCR4) expression are associated with dedifferentiation in neuroendocrine tumors (NET). In NET, CXCR4 expression is associated with enhanced metastatic and invasive potential and worse prognosis but might be a theragnostic target. Likewise, activation of Wnt/β-catenin signaling may promote a more aggressive phenotype in NET. We hypothesized an interaction of the Wnt/β-catenin pathway with CXCR4 expression and function in NET. The NET cell lines BON-1, QGP-1, and MS-18 were exposed to Wnt inhibitors (5-aza-CdR, quercetin, and niclosamide) or the Wnt activator LiCl. The expressions of Wnt pathway genes and of CXCR4 were studied by qRT-PCR, Western blot, and immunohistochemistry. The effects of Wnt modulators on uptake of the CXCR4 ligand [\(^{68}\)Ga] Pentixafor were measured. The Wnt activator LiCl induced upregulation of CXCR4 and Wnt target gene expression. Treatment with the Wnt inhibitors had opposite effects. LiCl significantly increased [\(^{68}\)Ga] Pentixafor uptake, while treatment with Wnt inhibitors decreased radiopeptide uptake. Wnt pathway modulation influences CXCR4 expression and function in NET cell lines. Wnt modulation might be a tool to enhance the efficacy of CXCR4-directed therapies in NET or to inhibit CXCR4-dependent proliferative signaling. The underlying mechanisms for the interaction of the Wnt pathway with CXCR4 expression and function have yet to be clarified.
Whole-Body [\(^{18}\)F]FDG PET/CT Can Alter Diagnosis in Patients with Suspected Rheumatic Disease

The 2-deoxy-d-[\(^{18}\)F]fluoro-D-glucose (FDG) positron emission tomography/computed tomography (PET/CT) is widely utilized to assess the vascular and articular inflammatory burden of patients with a suspected diagnosis of rheumatic disease. We aimed to elucidate the impact of [\(^{18}\)F]FDG PET/CT on change in initially suspected diagnosis in patients at the time of the scan. Thirty-four patients, who had undergone [\(^{18}\)F]FDG PET/CT, were enrolled and the initially suspected diagnosis prior to [18F]FDG PET/CT was compared to the final diagnosis. In addition, a semi-quantitative analysis including vessel wall-to-liver (VLR) and joint-to-liver (JLR) ratios was also conducted. Prior to [\(^{18}\)F]FDG PET/CT, 22/34 (64.7%) of patients did not have an established diagnosis, whereas in 7/34 (20.6%), polymyalgia rheumatica (PMR) was suspected, and in 5/34 (14.7%), giant cell arteritis (GCA) was suspected by the referring rheumatologists. After [\(^{18}\)F]FDG PET/CT, the diagnosis was GCA in 19/34 (55.9%), combined GCA and PMR (GCA + PMR) in 9/34 (26.5%) and PMR in the remaining 6/34 (17.6%). As such, [\(^{18}\)F]FDG PET/CT altered suspected diagnosis in 28/34 (82.4%), including in all unclear cases. VLR of patients whose final diagnosis was GCA tended to be significantly higher when compared to VLR in PMR (GCA, 1.01 ± 0.08 (95%CI, 0.95–1.1) vs. PMR, 0.92 ± 0.1 (95%CI, 0.85–0.99), p = 0.07), but not when compared to PMR + GCA (1.04 ± 0.14 (95%CI, 0.95–1.13), p = 1). JLR of individuals finally diagnosed with PMR (0.94 ± 0.16, (95%CI, 0.83–1.06)), however, was significantly increased relative to JLR in GCA (0.58 ± 0.04 (95%CI, 0.55–0.61)) and GCA + PMR (0.64 ± 0.09 (95%CI, 0.57–0.71); p < 0.0001, respectively). In individuals with a suspected diagnosis of rheumatic disease, an inflammatory-directed [\(^{18}\)F]FDG PET/CT can alter diagnosis in the majority of the cases, particularly in subjects who were referred because of diagnostic uncertainty. Semi-quantitative assessment may be helpful in establishing a final diagnosis of PMR, supporting the notion that a quantitative whole-body read-out may be useful in unclear cases.
Chronic hepatitis C can be treated very effectively with direct-acting antivirals (DAA) with only minor side effects compared to an interferon-containing treatment regimen. The significance of metabolic comorbidities after HCV cure is not well defined. This study aims to investigate short- and long-term weight change of patients receiving interferon-free antiviral treatment for chronic hepatitis C. The German Hepatitis C-registry (DHC-R) is a national multicenter real-world cohort. A total of 5111 patients were followed prospectively after DAA treatment for up to 3 years. Weight change compared to baseline was analyzed at end of treatment and at years 1, 2, and 3 after completion of antiviral therapy. Regression analysis was performed to identify baseline predictors for weight change. While there was no relevant mean weight change (−0.2 kg, SD 4.3 kg) at the end of antiviral treatment, weight started to increase during long-term follow-up reaching +1.7 kg (SD 8.0 kg, p < 0.001) compared to baseline at 3 years (follow-up year 3, FU3) after completion of antiviral therapy. 48%, 31%, and 22% of patients had a weight gain greater than 1, 3, and 5 kg at FU3, respectively. During follow-up, a body mass index (BMI) <30 proved to be the only consistent predictor for weight gain. DAA treatment is followed by a substantial weight gain (+3 kg or more) in one-third of the patients during long-term follow-up. Non-obese patients seemed to be most vulnerable to weight gain. The body compartment involved in weight gain as well as the mechanism of weight gain remain to be elucidated.
Primäre Nestin-positive adulte Stamm-/Vorläuferzellen aus menschlichen Langerhans'schen Inseln besitzen einen mesenchymalen Charakter und das prinzipielle Potenzial zur in vitro-Differenzierung in Insulin produzierende Phänotypen. Allerdings ist die Entwicklung effektiver Differenzierungsstrategien bisher noch nicht gelungen. Dies ist unter anderem durch das limitierte Wachstumsverhalten dieser Primärzellen in Kultur begründet, das in der vorliegenden Arbeit ausführlich charakterisiert wurde. So besitzt die Gesamtpopulation aus pankreatischen humanen Langerhansschen Inseln auswachsender Zellen (hIZ) ein begrenztes Wachstumspotenzial von im Mittel 19 Passagen. Diese Tatsache limitiert zum einen die Entwicklung von Protokollen zur Differenzierung dieser Zellen und führt zum anderen zu einer Limitierung der Vision in vitro vermehrbaren und differenzierbaren Vorläuferzellmaterials, das nach Differenzierung transplantiert werden und in vivo die beta-Zellfunktion ersetzen könnte. Vor diesem Hintergrund zeigt die vorliegende Arbeit anhand des Nestin-positiven und mesenchymalen Zellmodells der menschlichen Knochenmarksstammzelllinie hMSC-TERT weiterhin, dass sich eine gentechnisch induzierte transiente und stabile Überex-pression des wachstums- und proliferationsassoziierten Proteins p8 fördernd auf das Wachstumsverhalten dieser Zelllinie auswirkt. Dieser Effekt beruht, wie an stabil generierten p8-überexprimierenden Zelllinien gezeigt werden konnte, zum einen auf der Steigerung der Proliferationsrate. Zum anderen ist das verbesserte Wachstumsverhalten jedoch auch auf eine bis dato unbekannte Verminderung der basalen Apoptoserate von hMSC-TERT zurückzuführen. Das Protein p8 konnte erstmals als molekularer Mediator des Wachstums und Überlebens mesenchymaler Nestin-positiver und zu beta-Zellähnlichen Phänotypen differenzierbarer Vorläuferzellen charakterisiert werden. Es kann somit einen entscheidenden Beitrag zur Lösung des Problems begrenzten differenzierbaren Stammzellmaterials auf der Suche nach einer zellbasierten kurativen, breit und risikoarm einsetzbaren Therapiestrategie für den Diabetes mellitus leisten.
Myeloid-derived suppressor cells (MDSCs) represent a major population controlling T cell immune responses. However, little is known about their molecular requirements for homing and T cell interaction to mediate suppression. Here, we investigated the functional role of the homing and collagen IV receptor VLA-1 (α1β1-integrin) on in vitro GM-CSF generated murine MDSCs from wild-type (WT) and CD49a/α1-integrin (Itga1\(^{−/−}\)) gene-deficient mice. Here, we found that effector (Teff) but not naive (Tn) CD4\(^+\) T cells express VLA-1 and monocytes further up-regulated their expression after culture in GM-CSF when they differentiated into the monocytic subset of resting MDSCs (R-MDSCs). Subsequent activation of R-MDSCs by LPS+IFN-γ (A-MDSCs) showed increased in vitro suppressor potential, which was independent of VLA-1. Surprisingly, VLA-1 deficiency did not influence A-MDSC motility or migration on collagen IV in vitro. However, interaction times of Itga1\(^{−/−}\) A-MDSCs with Teff were shorter than with WT A-MDSCs on collagen IV but not on fibronectin substrate in vitro. After injection, A-MDSCs homed to the splenic red pulp where they co-localized with Teff and showed immediate suppression already after 6 h as shown by inhibition of T cell proliferation and induction of apoptosis. Injection of A-MDSCs from Itga1\(^{−/−}\) mice showed equivalent homing into the spleen but a reduced suppressive effect. Interaction studies of A-MDSCs with Teff in the subcapsular red pulp with intravital two-photon microscopy revealed also here that MDSC motility and migration parameters were not altered by VLA-1 deficiency, but the interaction times with Teff were reduced. Together, our data point to a new role of VLA-1 adhesion to collagen IV as a prerequisite for extended contact times with Teff required for suppression.
Adipositas und die hiermit verbundenen Folgen und Krankheitsbilder (wie Diabetes mellitus, Koronare Herzerkrankung etc.) haben sich in den letzten Jahren und Jahrzehnten zu einem globalen Problem entwickelt. Mögliche Therapieansätze und die Risikominimierung der Entstehung von Folgeerkrankungen (wie z.B. der Nichtalkholischen Fettlebererkrankung) sind somit immer weiter in den Fokus der Wissenschaft gerückt.
Da eine Korrelation zwischen hohem Körpergewicht und niedrigem Vitamin D-Spiegel mehrfach beschrieben wurde ebenso wie die positiven (u.a. antiinflammatorischen) Wirkungen des Vitamin D auf den menschlichen und murinen Organismus, beschäftigt sich die vorliegende Arbeit mit der Möglichkeit einer Vitamin D-Substitution als möglichen Therapieansatz zur Prävention und besseren Behandlung adipöser Folgeerkrankungen.
Zur Anwendung kam in der vorliegenden Arbeit ein Adipositas-Mäuse-Modell, wobei der Fokus auf der Untersuchung des epididymalen, weißen Fettgewebes lag. Verglichen wurden jeweils zwei Gruppen unter Niedrigfett- und Hochfett-Diät von denen jeweils eine Gruppe eine Hochdosis-Vitamin D3-Substitution erhielt. Das Fettgewebe wurde auf RNA- (mittels PCR) und Protein-Ebene (mittels ELISA und Western Blot) sowie auf mikroskopischer Ebene untersucht.
In der Gruppe der adipösen Tiere konnte bei den Tieren unter Hochfett- und Hoch-Vitamin D3-Diät ab Beginn der Substitution eine vermehrte Gewichtszunahme beobachtet werden. Einhergehend hiermit zeigten sich auf RNA-Ebene Hinweise auf eine vermehrte Hypoxie des Fettgewebes, die wir im Rahmen einer v.a. zunehmenden Hypertrophie als Mechanismus der Fettgewebszunahme werteten. Eine vermehrte Makrophagen-Einwanderung infolge der Hypoxie sowie ein Switch der Makrophagenpopulation in Richtung der pro-inflammatorischen M1-Makrophagen konnte beobachtet werden. Auf mikroskopischer Ebene bestätigte sich dies mit einer Zunahme an ‚crown like structures’, als morphologisches Korrelat der Ansammlung von M1-Makrophagen um die Adipozyten.
Als möglicher Ausdruck der vermehrten Fettgewebsinflammation konnte auch eine Zunahme an Insulinresistenz bei den Tieren unter Hochfett-Diät und Hoch-Vitamin D3-Substitution gezeigt werden. Neben erhöhten Glukose- und Insulinspiegeln im Serum der erwähnten Versuchsgruppe zeigten sich auch auf zellulärer Ebene eine Herabsetzung der Translation und Transkription von Genen der Insulinreiz-Antwort und Glukose-Aufnahme (IRS1 und GLUT4).
Zusammenfassend entsteht der Eindruck, dass die Vitamin D-Substitution im vorliegenden Untersuchungsmodell keinen positiven Effekt auf die niedriggradige Fettgewebsinflammation sowie auf die hierausfolgende Ausbildung einer Insulinresistenz hat. Als Ursache hierfür muss die gleich zu Beginn des Versuchsaufbaus auffallende vermehrte Gewichtszunahme unter Hochfett-Diät und Hochdosis-Vitamin D3-Substitution beim Vergleich der Hochfett-Gruppen gewertet werden. Der mögliche positive Effekt des Vitamin D3 scheint im untersuchten Zeitraum dem negativen Effekt des vermehrten Übergewichts nicht entgegenwirken zu können.
Background: Different parameters have been determined for prediction of treatment outcome in hepatitis c virus genotype 1 infected patients undergoing pegylated interferon, ribavirin combination therapy. Results on the importance of vitamin D levels are conflicting. In the present study, a comprehensive analysis of vitamin D levels before and during therapy together with single nucleotide polymorphisms involved in vitamin D metabolism in the context of other known treatment predictors has been performed.
Methods: In a well characterized prospective cohort of 398 genotype 1 infected patients treated with pegylated interferon-alpha and ribavirin for 24-72 weeks (INDIV-2 study) 25-OH-vitamin D levels and different single nucleotide polymorphisms were analyzed together with known biochemical parameters for a correlation with virologic treatment outcome.
Results: Fluctuations of more than 5 (10) ng/ml in 25-OH-vitamin D-levels have been observed in 66 (39) % of patients during the course of antiviral therapy and neither pretreatment nor under treatment 25-OH-vitamin D-levels were associated with treatment outcome. The DHCR7-TT-polymorphism within the 7-dehydrocholesterol-reductase showed a significant association (P = 0.031) to sustained viral response in univariate analysis. Among numerous further parameters analyzed we found that age (OR = 1.028, CI = 1.002-1.056, P = 0.035), cholesterol (OR = 0.983, CI = 0.975-0.991, P<0.001), ferritin (OR = 1.002, CI = 1.000-1.004, P = 0.033), gGT (OR = 1.467, CI = 1.073-2.006, P = 0.016) and IL28B-genotype (OR = 2.442, CI = 1.271-4.695, P = 0.007) constituted the strongest predictors of treatment response.
Conclusions: While 25-OH-vitamin D-levels levels show considerable variations during the long-lasting course of antiviral therapy they do not show any significant association to treatment outcome in genotype 1 infected patients.
Veränderungen im intestinalen Mikrobiom bei Patienten mit akuter Leukämie im longitudinalen Verlauf

In der vorliegenden Studie wurden Veränderungen des Darmmikrobioms anhand von Stuhlproben von Patienten mit akuter Leukämie longitudinal untersucht. Die Patienten wurden mit intensiver Chemotherapie behandelt. Die Therapie als auch die Erkrankung selbst führte zu einer erheblichen Immunsuppression der Patienten. Prophylaktisch und therapeutisch wurden intensive Antibiotikatherapien bei allen Patienten durchgeführt.
Das Mikrobiom wurde quantitativ und qualitativ analysiert. Die Bakterienmenge der Stuhlproben wurde mittels quantitativer Polymerase-Kettenreaktion und die Diversität des Mikrobioms mittels 16s rDNA Sequenzierung aufgezeigt. Zusätzlich dazu fand eine mikrobiologische Kultivierung von Bakterien in Rektalabstrichen statt, um multiresistente Keime nachzuweisen. Ebenso wurde der klinische Verlauf der Patienten dokumentiert.
Insgesamt wurde das Mikrobiom von drei verschiedenen Studiengruppen untersucht: Patienten mit akuter Leukämie, Patienten, die mit multiresistenten Keimen besiedelt waren und sich in der Nachsorge der Würzburger interdisziplinären onkologischen Tagesklinik befanden sowie gesunde Probanden.
Im Mikrobiom der Patienten mit akuter Leukämie war eine deutlich geringere Diversität sowie eine deutlich geringere Bakterienmenge im Vergleich zu beiden anderen Studiengruppen festzustellen. Das Mikrobiom änderte sich während des Therapieverlaufs erheblich und am Beispiel von einigen Patienten konnte gezeigt werden, dass einzelne Bakterien das Mikrobiom dominierten. Des Weiteren waren im Mikrobiom der Patienten mit akuter Leukämie mehr potenziell pathogene sowie weniger potenziell protektive Bakterien im Vergleich zur Kontrollgruppe vorhanden.
Zusammenfassend lässt sich sagen, dass sich das Mikrobiom der Patienten mit akuter Leukämie deutlich von dem der anderen Studiengruppen unterscheidet. Um die Daten zu validieren und einen eventuellen Einfluss des Mikrobioms auf das Überleben der Patienten zu identifizieren, sollten die Untersuchungen an einer deutlich größeren Studienpopulation wiederholt werden.
The known YAP inhibitor verteporfin is capable of repressing IL‐17A production in Th17 cells. However, this effect is mediated independently of YAP and can ameliorate Th17‐mediated experimental autoimmune encephalomyelitis (EAE) upon in vivo administration. The data suggest verteprofin's mode of action for the design of novel therapeutic autoimmune disease intervention.
Es ist schon lange bekannt, dass das Immunsystem eine wichtige Rolle in der Immunabwehr von malignen Tumoren spielt. In der vorliegenden Arbeit wurde das Verhalten der unten genannten Zellen, Rezeptoren und löslichem Protein MICA im peripheren Blut von Patienten mit soliden und hämatologischen Tumorerkrankungen sowie einer Kontrollgruppe mittels Durchflußzytometrie und ELISA-Verfahren untersucht. NK-Zellen ( u.a. CD 56+CD16-NK-Zellen, CD 56-CD16+ Nk-Zellen, CD56+CD16+NK-Zellen), NKT-Zellen und verschiedene T-Lymphozyten ( u.a. αβ-Lymphozyten, δγ-Lymphozyten, Vγ9Vδ1-Lymphozyetn, Vγ9Vδ2-Lymphozyten) sind auf direkten oder indirekten Weg über aktivierende Rezeptoren ( u.a. NKG2D, NKp44, NKp46, NKp33), inhibierende Rezeptoren ( u.a. p 58.1, p58.2, p70, NKG2A) und Chemokinrezeptoren ( u.a. CXCR1) an der Lyse / Apoptose von Tumorzellen über HLA-abhängige (MICA) und HLA-unabhängig Moleküle beteiligt. Es ließ sich zeigen, dass MICA genauso im Serum von hämatologischen Patienten wie auch bei soliden Tumorerkrankungen vorkommt, wobei vor allem Patienten mit fortgeschrittenen Stadien und Metastasierung hohe Werte aufwiesen. Es wurde eine Verminderung der Gesamtzahl an Lymphozyten und αβ-T-Lymphozyten bei hämatologischen und bei soliden Tumoren im peripheren Blut festgestellt. Weiterhin zeigte sich, wie bereits in der Literatur berichtet, bei hämatologischen Neoplasien ein signifikant erhöhter Wert an NK-Zellen und CD56-16+NK-Zellen sowie eine verminderte Expression von NKG2D auf NK-Zellen, auf αβ-T-Lymphozyten und auf γδ-T-Lymphozyten. Es ließ sich auch eine verminderte Expression von p58.1 und NKG2A auf γδ-Lymphozyten im Vergleich zur gesunden Kontrollgruppe nachweisen. Ein signifikanter Unterschied im Vergleich zu soliden Tumoren fand sich nicht, bis auf den oben genannten Lymphozytenanteil. Der Nachweis, dass erhöhte lösliche Serum MICA-Werte die Parameter wie KIRs und KARs sowie Chemokinrezetoren oder die Apopotose der Immunzellen bei hämatologischen oder bei soliden Tumorerkrankungen beeinflusst, konnte in dieser Arbeit nicht geführt werden. Die vorliegende Arbeit trägt dazu bei, die Immunabwehr gegenüber soliden und hämatologischen Tumorerkrankungen besser zu verstehen.
Aspergillus fumigatus (A. fumigatus) ist der häufigste Erreger der invasiven Aspergillose, welche vornehmlich immunsupprimierte Patientinnen und Patienten betrifft und mit einer hohen Letalität einhergeht. Zur Entwicklung neuer diagnostischer sowie therapeutischer Ansätze ist ein besseres Verständnis der Interaktion von A. fumigatus mit dem humanen Immunsystem zwingend erforderlich. Zur Erforschung dieser Interaktion werden häufig Mausmodelle herangezogen, welche aufgrund der unterschiedlichen Biologie des Wirts jedoch nicht direkt übertragbar sind. Ziel dieser Studie war es, einen funktionellen in vitro Vergleich zwischen humanen und murinen Makrophagen, neutrophilen Granulozyten (PMNs) und dendritischen Zellen (DCs) in ihrer Interaktion mit A. fumigatus Konidien, Keimschläuchen sowie depletiertem Zymosan zu erstellen, um eine bessere Beurteilung und Übertragbarkeit des Mausmodells bei der invasiven Aspergillose zu ermöglichen. Dabei wurden die verschiedenen Zellen des Immunsystems auf standardisierte und reproduzierbare Weise generiert und Stimulationsversuche durchgeführt.
Hierbei zeigten humane und murine Zellen in vitro eine unterschiedliche Antwort auf die Stimulation mit A. fumigatus: Murine Makrophagen und neutrophile Granulozyten zeigten im Vergleich zu den humanen Zellen eine stärkere primäre Immunantwort mit einer vermehrten Ausschüttung reaktiver Sauerstoffspezies (ROS). Humane DCs hingegen, welche als Bindeglied zwischen angeborenem und adaptivem Immunsystem fungieren, zeigten nach Stimulation mit A. fumigatus eine vermehrte Oberflächenexpression von Maturationsmarkern sowie eine höhere Phagozytoserate als die murinen DCs. Weiterhin konnte eine inverse Dectin-1-Expression auf humanen und murinen DCs nach Stimulation mit A. fumigatus nachgewiesen werden. Es konnte gezeigt werden, dass es für alle untersuchten Zelltypen Unterschiede zwischen humanen und murinen Zellen in der basalen und der Zytokinausschüttung nach Stimulation mit A. fumigatus gab.
In Zusammenschau der Ergebnisse dieser Arbeit zeigt das murine Immunsystem eine stärkere angeborene Immunantwort mit vermehrter ROS-Ausschüttung, jedoch auch eine anti-inflammatorische Zytokinantwort, um möglicherweise eine überschießende Inflammation zu verhindern. Dies könnte durch die stärkere Exposition der Maus gegenüber A. fumigatus durch den bodennahen Lebensraum sowie ihrer kurzen Lebensdauer bedingt sein. Im humanen System kommt hingegen der Aktivierung des adaptiven Immunsystems über die DCs eine übergeordnete Rolle zu. So zeigen beide Spezies distinkte Unterschiede in ihrer in vitro Immunantwort gegenüber A. fumigatus, welche bei der Übertragung von experimentellen Daten von der Maus auf den Menschen beachtet werden sollten.
Die Nukleosidischen Reverse Transkriptase Hemmer (NRTI) Zidovudin (AZT) und Stavudin (d4T) sind häufig eingesetzte Bestandteile der antiretroviralen Kombinationstherapie. Die Behandlung erstreckt sich oft über viele Jahre, sodass neben der antiviralen und immunologischen Effektivität besonders das Auftreten von Langzeitnebenwirkungen von Bedeutung ist. Im Rahmen dieser Arbeit wurden retrospektiv die Langzeit-Therapieverläufe von 213 Patienten, die zwischen 1990 und 2003 mit Zidovudin oder Stavudin behandelt wurden, verglichen. Die Kombinationsegime unterschieden sich nicht in ihrer antiretroviralen Wirksamkeit oder Einnahmedauer, jedoch in ihrem Nebenwirkungsprofil. So traten hämatologische Nebenwirkungen (Anämien, Leukopenien, Neutropenien) signifikant häufiger unter AZT auf. Die Gabe von Stavudin kann die hämatotoxische Wirkung von Zidovudin zum Teil kompensieren. Nach Therapieumstellung von AZT auf d4T kam es zu einem Anstieg der absoluten Leukozyten, der neutrophilen Granulozyten und des Hämoglobins. Sowohl in Zidovudin- als auch Stavudin-haltigen Regimen trat nach Beginn der antiretroviralen Therapie eine Makrozytose auf. Patienten mit Noncompliance zeigten eine anhaltende Normozytose bzw. eine Normalisierung des MCV, falls nach Beginn der ART eine Makrozytose bestand. Das MCV kann als Compliancemarker genutzt werden. Unter d4T-haltigen Regimen traten häufiger metabolische Nebenwirkungen wie Hypercholesterinämien, Hypertriglyceridämien und Hepatotoxizität auf, v.a. in Kombination mit Proteaseinhibitoren. Lipodystrophien wurden unter Proteasehemmer-haltigen und –freien Regimen beobachtet. Unter Stavudin traten Veränderungen der Körperfettverteilung signifikant häufiger auf als unter Zidovudin.
Eine klassische HIV-Primärtherapie setzt sich aus einer Kombination von zwei Nukleosidischen Reverse-Transkriptase-Inhibitoren (NRTI) mit Protease-Inhibitoren (PI) oder Nicht-Nukleosidischen Reverse-Transkriptase-Inhibitoren (NNRTI) zusammen. Vor allem aufgrund NRTI-assoziierter Nebenwirkungen durch mitochondriale Toxizität wird die Option eines Verzichts auf NRTI (NRTI-Sparing) untersucht. In der vorliegenden Langzeituntersuchung wurden drei therapienaive Patientenkollektive - (1) NRTI+PI [n=23], (2) NRTI+NNRTI [n=18] und (3) NNRTI+PI [n=19] - über 96 Wochen teilweise retrospektiv, teilweise prospektiv beobachtet. Die drei Kollektive wurden bezüglich Effektivität, Veränderungen von metabolischen und hämatologischen Laborparametern und kontinuierlichem Therapeutischen Drug Monitoring (TDM) anhand statistischer Auswertung verglichen. Dabei zeigten sich unter den NNRTI Efavirenz (EFV) und Nevirapin (NVP) geringere Plasmaspiegelschwankungen als unter PI. Im Vergleich der Therapiegruppen wurden für absolute und relative Plasmaspiegel keine signifikanten Unterschiede zwischen EFV, NVP und LPV (Lopinavir) beobachtet. Unabhängig des Krankheitsstadiums (CDC-Klassifikation) konnte in allen Gruppen eine maximale Suppression der Viruslast unter die Nachweisgrenze von <50 HIV-RNA-Kopien/ml und ein Anstieg der CD4-Zellzahlen beobachtet werden. Bezüglich virologischem und immunologischem Therapieerfolg gab es im Verlauf über 96 Wochen keinen signifikanten Unterschied zwischen den Therapiekollektiven. Im Vergleich der Laborparameter von Fett- und Leberstoffwechsel kam es zwischen den Gruppen zu signifikanten Unterschieden, wobei deutliche Anstiege und höchste Werte im NRTI-freien Kollektiv gemessen wurden. Für Hämoglobin wurde dies nicht beobachtet. NRTI-Sparing kann bei HIV-therapienaiven Patienten eine Option für eine effektive Langzeittherapie mit maximaler Virussuppression und ansteigender CD4-Zellzahl darstellen, scheint aber mit einem erhöhten Risiko für ansteigende Fettstoff- und Leberparameter assoziiert zu sein.
In dieser retrospektiven Datenanalyse wurde ein ungefiltertes Patientenkollektiv von 188 Patienten mit Erstdiagnose eines hepatozellulären Karzinoms im Zeitraum von 2005 bis 2012 am Universitätsklinikum Würzburg anhand verschiedener demografische, pathomechanistischer, diagnostischer, therapeutischer oder prognostischer Parameter untersucht. Durch die Datenanalyse konnten verschiedene prognostische Faktoren eruiert werden, die sich auf das mediane Überleben auswirken, wie zum Beispiel das Child-Pugh-Stadium, der AFP-Spiegel, die Pfortaderthrombose und das Geschlecht des Patienten. Darüber hinaus konnte gezeigt werden, dass Patienten von einem multimodalen Therapiekonzept profitieren.
Des Weiteren wurden Besonderheiten bei den Grunderkrankungen evaluiert. Die allermeisten HCCs sind im Würzburger Patientenkollektiv auf einen chronischen Alkoholabusus zurückzuführen. Abgesehen davon wird die Bedeutung der non-alkoholischen Fettlebererkrankung in den kommenden Jahren mehr und mehr zunehmen.
Background
Valid indicators are required to measure surgical quality. These ideally should be sensitive and selective while being easy to understand and adjust. We propose here the MTL30 quality indicator which takes into account 30-day mortality, transfer within 30 days, and a length of stay of 30 days as composite markers of an uneventful operative/postoperative course.
Methods
Patients documented in the StuDoQ|Colon and StuDoQ|Rectal carcinoma register of the German Society for General and Visceral Surgery (DGAV) were analyzed with regard to the effects of patient and tumor-related risk factors as well as postoperative complications on the MTL30.
Results
In univariate analysis, the MTL30 correlated significantly with patient and tumor-related risk factors such as ASA score (p<0.001), age (p<0.001), or UICC stage (p<0.001). There was a high sensitivity for the postoperative occurrence of complications such as re-operations (p<0.001) or subsequent bleeding (p<0.001), as well as a significant correlation with the CDC classification (p<0.001). In multivariate analysis, patient-related risk factors and postoperative complications significantly increased the odds ratio for a positive MTL30. A negative MTL30 showed a high specify for an uneventful operative and postoperative course.
Conclusion
The MTL30 is a valid indicator of colorectal surgical quality.
Background: With the availability of T-cell-directed therapy and next-generation compounds of established classes of drugs, the treatment of relapsed/refractory (r/r) myeloma is getting more complex. However, treatment options in practice are limited by availability, approval, and patient comorbidity. The aim of this article is to provide a practical approach toward the choice of treatment for r/r myeloma patients. Summary: Regarding market authorization and current guidelines, at least in Germany, most patients nowadays will have received a doublet or triplet combination as first-line therapy containing a proteasome inhibitor and an immunomodulatory drug, mostly lenalidomide. We focus on the treatment options for patients that are ineligible for (another) stem cell transplantation. We will review treatment options for relapse after first- or second-line therapy and beyond third-line. Key Messages: There is promising data supporting the efficacy and safety of triplet combinations containing anti-CD38-monoclonal antibodies (anti-CD38 mAbs) at first or second relapse in combination with next-generation compounds. For the treatment beyond third-line, comparative studies are scarce but some promising compounds are available via conditional authorization, and there is more to come in the future. We will present some early phase trials featuring promising results.
Die AML stellt mit einem Anteil von 80 % an den akuten Leukämien bei Erwachsenen eine bedeutende Erkrankung für die Gesellschaft dar. Aufgrund fehlender durchbrechender Erfolge in der Therapieentwicklung liegt die durchschnittliche Fünfjahresüberlebensrate dennoch nur bei etwa 25 %. Der Blick auf die Kraft des Graft-versus-Leukämie-Effekts nach allogener Stammzelltransplantation, eine Langzeitremission der AML erzielen zu können, weist jedoch auf die Immunogenität und Eignung der Erkrankung für neue immuntherapeutische Ansätze hin.
Anhand der Kartierung der in-vivo präsentierten MHC-Klasse-I-Peptidome auf
AML-Blasten sollten in dieser Arbeit potenziell geeignete Therapietargets identifiziert werden, um eine breitere Anwendung immuntherapeutischer Strategien bei AML-Patienten zu ermöglichen. Auf primären Patientenmaterialien, Zelllinien und benignen Zellen wurden hierzu über eine Immunoaffinitätschromatographie mit nachfolgenden Purifizierungsschritten
die MHC-präsentierten Peptide massenspekrometrisch-basiert
identifiziert. Zusätzlich erfolgte eine Quantifizierung der Oberflächen- und intrazellulären MHC-Klasse-I-Moleküle der verwendeten Proben durch einen indirekten Immunfluoreszenz-Assay.
Unter der Gesamtheit von 17.750 identifizierten nicht-redundanten MHC-Klasse-
I-präsentierten Peptiden konnte eine Vielzahl von 5.626 Peptiden mit Präsentationsfrequenzen bis zu 72 % als AML-exklusiv beschrieben werden. Hierunter wurden 240 kryptische Peptide vermeintlich nicht-codierenden Ursprungs identifiziert. Zudem wurden mehrere potenziell CMV-kreuzreaktive AML-Peptide erfasst, die zu der reduzierten Rezidivrate bei CMV-Infektion nach allogener Stammzelltransplantation führen könnten. Bei der MHC-Quantifizierung wiesen die AML-Blasten keine verminderte MHC-Expression auf und stellten sich somit als geeignete Target-Zellen für eine T-Zell-Immuntherapie dar.
Adoptive Immuntherapien mit allogenen Vγ9Vδ2 T-Zellen sind eine vielversprechende therapeutische Behandlungsstrategie für eine Reihe von hämatologischen Erkrankungen. Im Gegensatz zu konventionellen αβ T-Zellen sind allogene Vγ9Vδ2 T-Zellen in der Lage Tumorzellen MHC-unabhängig zu lysieren ohne eine „graft-versus-host“ (GvH)-Reaktion zu induzieren.
In der vorliegenden Arbeit wurde die in vitro Antileukämieantwort von HLA-inkompatiblen Vγ9Vδ2 T-Zellen gegenüber primären AML-Zellen systematisch untersucht. Die antileukämische Aktivität von Vγ9Vδ2 T-Zellen wurde in einem durchflusszytometrisch-basierten Zytotoxizitätsassay bestimmt und mit der Oberflächenexpression Killer-aktivierender und inhibierender Liganden (z.B. NKG2D- und DNAM1-Liganden), KIR-Liganden-Inkompatibilität zwischen Patienten und Spender und intrinsischen AML-Merkmalen (Zytogenetik, Immunphänotyp, Chemotherapiesensitivität der AML-Blasten) korreliert. Die beobachtete Zytotoxizität war deutlich heterogen (2.91 %- 56.26 %). 37 % der AML-Zellen waren primär empfindlich bzw. 63 % refraktär gegenüber Vγ9Vδ2 T-Zellen. Die Suszeptibilität der AML-Blasten gegenüber Vγ9Vδ2 T-Zellen korrelierte mit der Oberflächenexpression von ULBP1 und CD112 und monozytärer bzw. monoblastischer AML-Differenzierung. Die antileukämische Aktivität von Vγ9Vδ2 T-Zellen war dagegen unabhängig vom KIR-Liganden-Status zwischen Patienten und Spendern, zytogenetischem Risiko und Chemotherapiesensitivität der AML-Blasten. Die Vorbehandlung der Leukämiezellen mit Aminobisphosphonaten (Zoledronat) führte, insbesondere bei myelo-monozytär-differenzierten AML-Zellen, zu einer signifikanten dosisabhängigen Steigerung der antileukämischen Aktivität von Vγ9Vδ2 T-Zellen. Die Empfindlichkeit von myelo-monozytär-differenzierten Leukämiezellen gegenüber Zoledronat bzw. Vγ9Vδ2 T-Zellen korrelierte mit der Aktivität des Mevalonatmetabolismus. Dagegen zeigte die Mehrheit myeloblastischer AML-Blasten keine natürliche und nur geringe Aminobisphosphonat-induzierte Suszeptibilität gegenüber Vγ9Vδ2 T-Zellen. In der vorliegenden Arbeit konnten biologische Merkmale von AML-Blasten identifiziert werden, die mit der Antileukämieantwort von Vγ9Vδ2 T-Zellen korrelieren.
Bei Patienten mit Erkrankungen des blutbildenden Systems ist die hämatopoetische Stammzelltransplantation (HSZT) eine häufig eingesetzte kurative Therapie. Im Rahmen dieser Transplantation werden nicht nur vom Spender gewonnene hämatopoetische Stammzellen auf den Empfänger übertragen, sondern immer auch im peripheren Blut vorhandene T-Zellen. Dies kann zum einen einen positiven Effekt zum anderen aber auch negative Folgen für den transplantierten Patienten mit sich bringen. Eine negative Auswirkung wäre die sogenannte Graft-vesus-Host Disease (GvHD), bei der die T-Zellen des Spenders Zellen des Empfängers als fremd erkennen und angreifen. Klinisch manifestiert sich dies vor allem an Leber, Haut und Darm mit Ikterus, Dermatitiden und Diarrhoen. Einen gewünschten Effekt, den die übertragenen T-Zellen vor allem bei Patienten mit akuter myeloischer Leukämie (AML) mit sich bringen können, ist der sogenannte Graft-versus-Leukemia (GvL) Effekt. Dabei richten sich vom Spender stammende Immunzellen gegen die Tumorzellen des Empfängers und senken damit das Rezidivrisiko der Leukämie.
In verschiedenen Studien konnte eine positive Korrelation von CMV-Reaktivierung nach HSZT und einem niedrigerem Rezidivrisiko der hämatopoetischen Grunderkrankung gezeigt werden. Diese Doktorarbeit widmet sich auf Grundlage dessen der Frage, ob Cytomegalievirus (CMV)-spezifische cytotoxische T-Zellen (CTL) direkt durch Kreuzreaktivität zum GvL-Effekt beitragen.
Zunächst wurden periphere mononukleäre Zellen (PBMC) aus dem Blut neun gesunder Spender isoliert, die als CMV-seropositiv ausgetestet wurden. Diese wurden mit dem CMVpp65-(NLVPMVATV)-Einzelpeptid stimuliert und in Kultur angereichert. Zusätzlich wurden die expandierten CMV-spezifischen CTL durch eine spezifische Selektion über den Aktivierungsmarker CD137 weiter angereichert. Nach Expansion und Anreicherung zeigten jeweils 75% (Spender 1), 67% (Spender 2), 74% (Spender 3), 86% (Spender 4), 81% (Spender5), 80% (Spender 6), 84% (Spender 7), 51% (Spender 8) und 69% (Spender 9) der CD3+/CD8+-T-Zellen eine IFN-γ-Produktion und CD107a-Expression nach Stimulation mit dem CMVpp65-Einzelpeptid. IFN-γ als Effektormolekül der zytotoxischen Granula der CTL und CD107a als Degranulationsmarker beweisen die spezifische Zytotoxizität. Somit konnte die erfolgreiche Anreicherung funktionsfähiger CMVpp65-spezifischer CTL gezeigt werden. Um zu untersuchen, ob diese nun kreuzreaktiv tumorassoziierte Antigene (TAA) erkennen, wurden sie ebenfalls mit folgenden TAA stimuliert: WT1, Proteinase 3, PRAME, NY-ESO, Muc1 und Bcl-2. Die Stimulation erfolgte entweder über die direkte Zugabe von Einzelpeptiden bzw. Peptidpools oder über die Beladung und Präsentation dieser Peptide bzw. Peptidpools über dendritische Zellen (DC). Die DC wurden aus Monozyten des jeweiligen Spenders generiert. Im Falle von drei Spendern zeigt sich ebenfalls eine deutliche zytotoxische Funktion nach Stimulation mit dem WT1-(DFKDCERRF)-Einzelpeptid durch IFN-γ-Produktion und CD107a-Expression bei 75% (Spender 1), 35% (Spender 4) und 33% (Spender 7) der CD3+/CD8+-T-Zellen. Wie zuvor erwähnt lag der Anteil der CD3+/CD8+-T-Zellen mit spezifischer Zytotoxizität nach Stimulation mit dem CMVpp65-(NLVPMVATV)-Einzelpeptid bei diesen drei besagten Spendern bei 74% (Spender1), 86% (Spender 4) und 84% (Spender7). So ergab sich für diese drei Spender eine gemeinsame Schnittmenge von 48,92% (Spender 1), 21,07% (Spender 4) und 17,45% (Spender 7) derjenigen Zellen, die sowohl nach Stimulation mit CMVpp65-(NLVPMVATV)-Einzelpeptid und WT-(DFKDCERRF)-Einzelpeptid eine zytotoxische Funktion zeigten, sodass von einer kreuzreaktiven Erkennung dieser beiden Peptide in diesen drei Spendern ausgegangen werden muss. Die für diese Spender gezeigte kreuzreaktive Erkennung könnte zum GvL-Effekt bei Leukämie/Myelom-Patienten nach HSZT beitragen.
In der vorliegenden Arbeit wurden die Behandlungsabläufe von Patienten mit chronischer Hepatitis C unter Therapie mit Standard-Interferon (Kollektiv 1) bzw. mit pegyliertem Interferon (Kollektiv 2) ausgewertet. Die meisten Patienten erhielten eine Kombinationstherapie mit Ribavirin. Es bestand Strukturgleichheit für die Kollektive hinsichtlich Alter, Geschlecht, BMI vor Therapie, Übertragungsweg der Hepatitis, Hepatitis B-Infektion, HAI-Grading-Score und HAI-Staging-Score. Ein signifikanter Unterschied bestand für das Merkmal HIV-Koinfektion. Nach Therapiebeginn zeigte sich ein schnell einsetzendes serologisches und virologisches Ansprechen. Patienten unter Therapie mit pegyliertem Interferon und Ribavirin hatten die besten Chancen auf ein anhaltendes Therapieansprechen. Eine early virological Response war ein guter Prädiktor für das Erreichen einer sustained virological Response. Die meisten Patienten berichteten über Nebenwirkungen unter Therapie. Die häufigsten Nebenwirkungen waren Müdigkeit und Schmerzen, v.a. in Form von Kopfschmerzen. Diese kamen jeweils bei ca. 70% der Patienten vor. Eine Anämie trat bei ca. 9% der Patienten auf. Hämatokrit, Hämoglobin und Erythrozyten sanken im Kollektiv 2 stärker ab als im Kollektiv 1. Unter Kombinationstherapie mit Ribavirin sank das Hämoglobin zudem mehr ab als unter Interferon- Monotherapie, was auf den hämolytischen Effekt des Ribavirins zurückzuführen ist. Thrombozyten fielen unter Kombinationstherapie im Kollektiv 1 deutlich geringer ab als im Kollektiv 2, was durch einen stärkeren myelosuppressiven Effekt des pegylierten Interferons bedingt sein könnte. Im Kollektiv 1 sanken die Thrombozyten unter Monotherapie stärker ab als unter Kombinationstherapie. Leukopenien traten häufiger unter Therapie mit pegyliertem Interferon auf. Insgesamt zeigte sich im hier analysierten Kollektiv ein geringes Risiko für eine Neutropenie oder Lymphopenie. Vor allem ältere Patienten mit niedrigen neutrophilen Granulozyten bzw. Lymphozyten vor Therapie schienen ein erhöhtes Risiko für eine Neutropenie bzw. Lymphopenie zu haben. Das Therapieansprechen und die Therapiedauer waren für Patienten mit bzw. ohne Leukopenie, Neutropenie oder Lymphopenie ähnlich. Für Infektionen fand sich ebenfalls kein signifikant erhöhtes Risiko bei Patienten mit Leukopenie, Neutropenie oder Lymphopenie. 16% der Patienten im Gesamtkollektiv hatten eine Infektion unter Therapie. Es zeigte sich kein Unterschied zwischen Kollektiv 1 und 2 für Infektionen unter Therapie. Patienten mit bzw. ohne Infektion wurden hinsichtlich der Merkmale Alter, Geschlecht, BMI, Hepatitis B-Infektion, Hepatitis G/GB-Infektion und HIV-Infektion verglichen. Zudem wurden die prätherapeutischen Laborwerte Ferritin, Viruslast, Eisen, TSH, GPT, GOT, Hämoglobin, Hämatokrit, Leukozyten, neutrophile Granulozyten, Lymphozyten, Thrombozyten und Erythrozyten gegenübergestellt und Therapiedauer und Therapieansprechen für Patienten mit bzw. ohne Infektion erhoben. Für keines dieser Kriterien lag ein signifikanter Unterschied zwischen Patienten mit Infektion und Patienten ohne Infektion vor. Die meisten Infektionen waren unkomplizierte, respiratorische Infektionen. Diese traten für Patienten mit Neutropenie und Patienten ohne Neutropenie gleich häufig auf. HIV-Patienten hatten ein höheres Infektionsrisiko. Jedoch war der Unterschied nicht signifikant. Beim Vergleich des prozentualen Absinkens der Lymphozyten vom Ausgangswert zeigte sich ein schwach signifikanter Unterschied zwischen den Werten zu Infektionszeitpunkten und den Werte für Patienten ohne Infektion. Für die absoluten Werte war der Unterschied nicht signifikant. Für neutrophile Granulozyten und Leukozyten fanden sich keine Unterschiede zwischen den Werten für Infekt-Patienten zum Infektionszeitpunkt, den Werten zu infektfreien Zeitpunkten und den Werten für Patienten ohne Infektion. Insgesamt fand sich im hier untersuchten Kollektiv keine Assoziation von Infektionen unter Interferontherapie mit Leukopenien oder Neutropenien. Ein Absinken der neutrophilen Granulozyten scheint daher in größerem Maße ohne Dosisreduktion tolerierbar zu sein als bisher empfohlen. Ein relativer Lymphozytenmangel könnte mit dem Auftreten von Infektionen assoziiert sein. Für den absoluten Lymphozytenmangel fand sich diese Assoziation jedoch nicht.
Durch die im Labor der AG Topp standardisierte Verfahren konnten im Rahmen dieser Arbeit stabile T Zell Linien aus primär humanen CMV spezifischen T Zellen und CMV spezifischen T Zellen mit chimärischen Rezeptoren hergestellt werden. Die Anzahl der chimärischen Rezeptoren auf den unterschiedlichen T Zellinien ist nicht signifikant unterschiedlich und beträgt ca. 17000 Rezeptoren pro Zelle. Bei der Überprüfung des Lyseverhaltens CMV spezifischer T Zellen gegenüber CMV spezifischer T Zellen transduziert mit einem chimärischen Rezeptor zeigt sich das gleiche Lyseverhalten. Um die zeitliche Dynamik dieses Verhaltens besser darzustellen, wurde ein FACS basierter Lysierungsversuch selbstständig entwickelt und etabliert. Somit konnte herausgestellt werden, dass das Lyseverhalten der T Zelllinien sowohl der CMV spezifischen T Zellen als auch das der Erst- und Zweitgeneration- T Zelllinien CAR CD19z und CAR CD19/28z vergleichbar war. Alle genannten Zelllinien lysieren ihre Targetzellen sowohl in der Quantität als auch in der zeitlichen Dynamik vergleichbar. Dies ist die einzige Gemeinsamkeit der verglichenen T Zell Aktivierungssysteme. In allen anderen funktionellen Versuchen ist die Aktivierung der T Zellen über einen chimärischen Rezeptor dem der physiologischen Aktivierung über den T Zell Rezeptor unterlegen. Der Proliferationsversuch mit CFSE zeigt eindeutig, dass nur die T Zellen, die über den TCR aktiviert werden, das Potential haben, sich zu teilen. Die T Zellen, die über den CAR sowohl der ersten als auch der zweiten Generation aktiviert werden, teilen sich nicht. Der Defekt der CAR T Zellen zeigt sich auch in der Hochregulation von CD25, einem Aktivierungsmarker für T Zellen. Die T Zellen, die durch die chimärischen Rezeptoren sowohl der ersten als auch der zweiten Generation aktiviert werden, regulieren CD25 nicht so stark nach oben wie die T Zellen, die physiologisch durch den TCR aktiviert werden. Diese Minderaktivierung spiegelt sich auch in der Zytokinproduktion wieder. Auch hier zeigt sich die unvollständige Aktivierung der CAR T Zellen. In der intrazellulären Zytokinmessung sieht man, dass bei CAR T Zellen der ersten und zweiten Generation zum einen die Einzelzellen weniger IFNy produzieren, zum anderen die Gesamtzahl der Zellen, die Zytokine produzieren, weniger ist als im Vergleich zur TCR Aktivierung. Dieses Phänomen der defizienten Aktivierung und der funktionellen Defekte von CAR T Zellen ist bekannt. Dies konnte noch einmal unter standardisierten und kontrollierten Bedingungen bestätigt werden. Durch die relativ neue Methode der durchflusszytometrischen Bead Technologie konnte dies nun zum ersten Mal realisiert werden. Es zeigte sich, dass ein signifikanter Unterschied zwischen den verschiedenen Aktivierungsmodi der T Zellen besteht. Eine physiologische Aktivierung von T Zellen führt zu einer höheren Maximum-Phosphorylierung als eine Aktivierung über den Erstgeneration CAR. Der signifikante Unterschied zeigt sich bei den MAP Kinasen ERK, JNK und p38. Somit ist dies ein Hinweis, dass die Signaltransduktion auf breiter Ebene aufgrund des alleinigen Vorhandenseins der Zeta Kette bei den CAR CD19z T Zellen, defizient ist und nicht ausreicht für eine volle Aktivierung, die eine volle Funktionalität der T Zelle nach sich ziehen würde. Diese unzureichende Aktivierung soll durch die kostimulatorische Komponente CD28 beim Zweitgenerationrezeptor CAR CD19/28z aufgehoben werden. In der Proliferation, Zytokineproduktion und den Aktivierungsmarkern zeigt sich keine Verbesserung im Vergleich zum Erstgenerationrezeptor bei CMV spezifischen T Zellen. Dieses Ergebnis wird bestätigt und korreliert damit, dass es keinen Unterschied in der maximalen Phosphorylierung von ERK, JNK und p38 zwischen Erstgeneration CAR CD19z und Zweitgeneration CAR CD19/28z gibt. Somit ist dieses System geeignet, um aus dem Phosphorylierungsstatus von MAP Kinasen von CAR T Zellen auf die Funktionalität dieser T Zellen zu schließen.
Das multiple Myelom, eine maligne Plasmazell-Dyskrasie, ist bis heute unheilbar. Eine Verbesserung des medianen Überlebens von weniger als 2 auf über 3 Jahre wurde erstmals 1996 durch eine prospektiv randomisierte Studie von Attal et al. gezeigt. Die retrospektive Analyse zweier zeitlich definierter Kohorten, Kohorte 1 (1990-1996) und Kohorte 2 (1997-2002), zur Überprüfung des Therapieerfolges am Universitätsklinikum Würzburg zeigt unter Berücksichtigung aller Patienten keinen Überlebensvorteil für Patienten der 2.Kohorte. Signifikant profitiert haben allerdings Patienten bis max. 65 Jahre der Kohorte 2, die durch die HD-Therapie ein 5-JÜL von 50% (Kohorte 2) vs.32% (Kohorte 1) erreichten, wohingegen sich für ältere Patienten keine signifikant messbaren Überlebensvorteile ergaben.
HINTERGRUND: Die Leber nimmt bei der Regulierung und Aufrechterhaltung des Uridinplasmaspiegels eine zentrale Rolle ein. Die Synthese von Uridin hängt dabei wesentlich von der intakten Funktion hepatozytärer Mitochondrien und des mitochondrialen Enzymes Dihydroorotatdehydrogenase (DHODH) ab. Bei Patienten unter HIV-Therapie zeigten sich in Studien verminderte Uridinplasmaspiegel, wobei die dafür verantwortlich gemachte therapieassoziierte mitochondriale Toxizität durch Uridinsupplementation reduziert werden konnte. Schädigungen von Leber und Mitochondrien im Rahmen chronischer Lebererkrankungen wie Hepatitis C (CHC), B (CHB), alkoholischer und nichtalkoholischer Fettleber (AFLD und NAFLD) könnten ebenfalls zu einem Abfall des Uridinplasmaspiegels führen. METHODIK: Nach Etablierung einer HPLC-Methodik zur Evaluation von Uridin im menschlichen Serum wurden Uridinplasmaspiegel von Patienten mit oben genannten Erkrankungen mit denjenigen einer gesunden Kontrollgruppe verglichen. Ferner wurden die Spiegel mit Demografie, Genotypen, Viruslasten, antiviraler Therapie und Therapiedauer sowie mit sonographischen wie histologischen Leberbefunden und Laborparametern korreliert. ERGEBNISSE: Sämtliche 130 Patienten zeigten einen signifikant niedrigeren Uridinplasmaspiegel als die aus 14 Probanden bestehende gesunde Kontrollgruppe, lagen jedoch noch im physiologischen Normbereich gesunder Erwachsener. In absteigender Reihenfolge zeigten die grössten Unterschiede die Gruppen mit chronischer Hepatits C (n = 69), Hepatitis B (n = 37) sowie AFLD/AFLD (n = 24). Demographische Faktoren, chronischer Alkoholkonsum, histologischer Grad der Leberentzündung und -fibrose, sowie Diabetes mellitus zeigten keinen Einfluss. Spezifika der Virushepatitiden zeigten, abgesehen von der Viruslast bei CHC mit Tendenz zu eher niedrigen Uridinplasmasppiegeln bei hohen Lasten ebenfalls keine Signifikanzen. Eine Leberverfettung zeigte hinsichtlich des Uridinplasmaspiegels lediglich im Vergleich der Gruppe AFLD/NAFLD zur Kontrollgruppe signifikant reduzierte Werte. In Bezug auf das absolute wie relative Vorliegen einer Leberzirrhose zeigten sich ebenfalls signifikant erniedrigte Spiegel, und auch bei Korrelation des Child-Pugh-Index sowie laborchemischen Lebersynthesemarkern zeigten sich mit Zunahme der Leberschädigung reduzierte Uridinspiegel. SCHLUSSFOLGERUNG: Der Uridinplasmaspiegel scheint bei chronischen Lebererkrankungen wie Hepatitis C und B sowie alkoholischer und nichtalkoholischer Fettleber relativ erniedrigt zu sein, ebenso bei der Leberzirrhose. Um den Einfluss einer Mitochondrienschädigung genauer einzuordnen wären zukünftige Untersuchungen unter Einbezug oxidativer Marker ebenso interessant wie Überlegungen, Uridin bei fortgeschrittenen chronischen Lebererkrankungen zu substituieren.
In der vorliegenden Untersuchung wurden die Serumspiegel der antiretroviralen Medikamente Nevirapin, Efavirenz und Lopinavir bei 124 ambulanten HIV-Patienten der HIV-Ambulanz des Bugando Hospitals in Mwanza (Tansania) bestimmt. Sechs Patienten mussten ausgeschlossen werden, so dass letztlich bei 118 Patienten überprüft wurde, welche Serumspiegel sich zu zufällig gewählten Zeitpunkten während einer Routineuntersuchung einstellten und welche Einflussfaktoren sich auf die Serumspiegel auswirkten.
Insgesamt 11 % der Patienten, die NVP und EFV eingenommen hatten, wiesen Serumspiegel auf, die im subtherapeutischen Bereich lagen. 11 % der Patienten, die EFV eingenommen hatten, wiesen Spiegel auf, die oberhalb des therapeutischen Bereiches lagen. 26 % der Patienten wiesen hohe NVP-Spiegel > 6.000 ng/ml auf, die jedoch aufgrund fehlender Daten für einen oberen cut-off-Wert nicht eindeutig einzuordnen sind. Die Patienten, die LPV eingenommen hatten, wiesen suffiziente Serumspiegel auf. Der Großteil der Patienten lag somit in dem anzustrebenden Bereich der Serumspiegel.
Die häufigsten Gründe für Spiegel außerhalb des therapeutischen Bereiches waren eine eingeschränkte Adhärenz und wahrscheinlich Medikamenteninteraktionen mit Tuberkulostatika.
Die Serumspiegel von NVP waren bei subjektiv angegebener eingeschränkter Adhärenz signifikant niedriger (p=0,026) und auch signifikant häufiger subtherapeutisch (p=0,005) womit gezeigt werden konnte, dass schon eine einmalig vergessene Einnahme zu niedrigeren und sogar subtherapeutischen Spiegeln führen kann. Dieser Zusammenhang unterstreicht die ausgesprochene Wichtigkeit einer guten Adhärenz.
11 % der Patienten wiesen Medikamentenspiegel auf, die oberhalb des therapeutischen Bereiches lagen, welche aber nur selten mit Nebenwirkungen einhergingen. Im Falle von NVP konnte ein marginal signifikanter Zusammenhang zwischen hohen NVP-Spiegeln und erhöhten GPT-Werten gezeigt werden. Diese Ergebnisse verdeutlichen die Schwierigkeit, einen klaren therapeutischen Bereich zu definieren, und zeigen, dass weitere Untersuchungen notwendig sind, um den Nutzen von TDM zur Vermeidung von Nebenwirkungen zu klären.
Zwischen höherem Körpergewicht und niedrigeren EFV-Serumspiegeln konnte eine marginal signifikante negative Korrelation gezeigt werden, so dass bei Patienten mit sehr hohem oder sehr niedrigem Körpergewicht TDM sinnvoll erscheint.
Zusammenfassend scheint der routinemäßige Einsatz von TDM auch bei tansanischen Patienten nicht notwendig zu sein. Bei gewissen Patienten kann TDM aber sinnvoll sein, vor allem um subtherapeutische oder toxische Spiegel zu vermeiden. Hilfreich scheint der Einsatz von TDM bei Patienten mit Adhärenzproblemen, einer Komedikation mit Rifampicin und Isoniazid und eventuell bei über- oder unterdurchschnittlichem Körperwicht und EFV-Therapie sowie erhöhten Transaminasen unter NVP-Therapie.
Höheres Alter, Krankheitsprogression, auffällige CD4-Zellzahlen oder eine schon lange andauernde ART scheinen keine eigenständige Indikationen für ein TDM darzustellen, da die meisten Patienten mit diesen Kriterien suffiziente Serumspiegel aufwiesen.
Die signifikant niedrigeren Serumspiegel der eingeschränkt adhärenten Patienten in dieser Untersuchung im Vergleich zu Patienten, die gute Adhärenz angegeben hatten, unterstreichen die Wichtigkeit einer gründlichen Aufklärung der Patienten über die Notwendigkeit einer guten Adhärenz.
HINTERGRUND: Langzeitnebenwirkungen der antiretroviralen Therapie werden u.a. auf mitochondriale Toxizität von NRTIs zurückgeführt, wobei es zu einer Dysfunktion der Atmungskette und nachgeschalteter Stoffwechselwege wie der de-novo-Pyrimidinsynthese kommen kann. Experimentell konnte gezeigt werden, dass das Pyrimidinnukleosid Uridin einige dieser Toxizitätseffekte reduzieren konnte. METHODIK: Nach Etablierung einer HPLC-Methodik zur Evaluation von Uridin im menschlichen Serum wurden Uridinspiegel von HIV-positiven Patienten mit Uridinspiegeln einer HIV-negativen Kontrollgruppe verglichen sowie mit Demografie, Nebenwirkungen bzw. Koerkrankungen, antiretroviraler Therapie und Therapiedauer korreliert. ERGEBNISSE: Die 182 HIV-Patienten wiesen signifikant niedrigere Uridinspiegel als die 30 Kontrollpersonen auf, blieben aber noch im physiologischen Normbereich gesunder Erwachsener. Demografische Faktoren sowie Marker der HIV-Infektion zeigten keine signifikante Korrelation mit Uridin. HIV-Patienten mit Begleiterkrankungen boten im Vergleich zu HIV-Patienten ohne Begleiterkrankungen signifikant verminderte Uridinspiegel. Bei Differenzierung nach den einzelnen Befunden zeigte sich, dass nur Patienten mit Diabetes mellitus Typ 2 und chronischen Lebererkrankungen wie Steatosis Hepatis signifikant erniedrigte Spiegel aufwiesen. Im Vergleich von 4 HIV-Patientengruppen mit unterschiedlicher antiretroviraler Therapie verhielten sich die Uridinspiegel analog zur potenziellen mitochondrialen Toxizität des jeweiligen Therapieregimes. Die Patientengruppe mit dideoxy-NRTI-haltiger Therapie wies die niedrigsten Uridinplasmaspiegel auf und zeigte im Vergleich zur Patientenguppe ohne antiretrovirale Therapie signifikant erniedrigte Uridinplasmaspiegel. Mit zunehmender Therapiedauer zeigte sich bei der Therapiegruppe mit dideoxy-haltiger HAART abfallende, bei den anderen 3 Therapiegruppen konstante bis leicht ansteigende Spiegel. SCHLUSSFOLGERUNG: Bei HIV-positiven Patienten scheint die Einnahme von (di)deoxy-NRTIs sowie Koerkrankungen wie Fettleber und Diabetes mellitus Typ 2 mit signifikant niedrigeren Uridinspiegeln assoziiert zu sein. Ob eine Uridinsubstitution zur Vermeidung von NRTI-assoziierten Langzeitnebenwirkungen sinnvoll ist, müssen weitere Untersuchungen zeigen.
Eine Infektion mit dem humanen Cytomegalievirus ist immer noch eine der häufigsten und bedrohlichsten Komplikationen nach einer allogenen Stammzelltransplantation (SCT), welche eine hohe Morbidität und Mortalität verursacht. Die prophylaktische oder preämptive antivirale Chemotherapie konnte den frühen Ausbruch einer CMV-Erkrankung während der ersten 100 Tage nach SCT signifikant reduzieren, jedoch kommt es dadurch häufig zu einem späten Ausbruch der CMV-Erkrankung und schwerwiegenden Nebenwirkungen wie Myelotoxizität und Nephrotoxizität. Zur Bekämpfung und Langzeitkontrolle einer CMV-Infektion ist eine effiziente zellvermittelte CMV-spezifische Immunität unabdingbar. Im Rahmen dieser Dissertation, wurden deshalb drei CMV-Vakzinkandidaten basierend auf dem hoch attenuierten Modifizierten Vaccinia Ankara Virus (MVA), welche stabil pp65 und/oder IE1 (MVA-IE1, MVA-pp65, and MVA-IE1-pp65) exprimieren und zugleich frei von Selektionsmarkern sind, auf ihre Fähigkeit hin untersucht CMV-spezifische T-Zellantworten zu induzieren. Als erstes wurden humane mononukleäre Zellen des periphären Blutes (PBMCs) und Leukozytensubpopulationen (aus Monozyten generierte dendritische Zellen (DCs), Monozyten und B-Zellen) mit MVA infiziert um deren Infektionsrate, Veränderungen in der Expression der Oberflächenmarker und der Zytokinexpression sowie deren Apoptoserate zu untersuchen. Monozyten, DCs und B-Zellen waren besonders empfänglich für eine MVA-Infektion, gefolgt von NK-Zellen. Monozyten wurden stark aktiviert, was sich durch eine erhöhte Expression der kostimulatorischen Moleküle, MHC-Komplexe und CCR7 zeigte, wohingegen DCs eine inkomplette Aktivierung vorwiesen und B-Zellen gehemmt wurden. Des Weiteren wurde die Expression von CXCL10, TNFa, IL-6 und IL-12 signifikant in den Antigen-präsentierenden Zellen (APCs) erhöht, aber die von IL-1b und IL-10 blieb unverändert oder wurde sogar signifikant reduziert. MVA induzierte also eine Th1-polarisierenden Zytokinexpression in den APCs. Allerdings konnten CMV-spezifische T-Zellen nicht mit direkter Antigenpräsentation durch DCs expandiert werden, da die DCs nach Infektion mit MVA schnell durch Apoptose starben und eine unzureichende Expression der kostimulatorischen Moleküle und MHC-Komplexe aufwiesen. Vielmehr konnte gezeigt werden, dass die erfolgreiche Expansion CMV-spezifischer T-Zellen mittels Kreuzpräsentation von Antigenen MVA-infizierter Leukozyten durch DCs erfolgte. Die Phagozytose von apoptotischen Material von MVA-infizierten Leukozyten mit anschließender Antigenprozessierung induzierte eine vollständige Ausreifung der DCs in vitro einhergehend mit erhöhter IL-12-Expression, was erheblich zu einer erfolgreiche T-Zell-Stimulation und –Expansion beitrug. Neben pp65-spezifischen T-Zellen wurden auch IE1-spezifische T-Zellen expandiert, wenn auch in einem geringeren Ausmaß. Der größte Teil der expandierten T-Zellen wies einen Effektor-Gedächtnis-(EM)-Phänotyp auf. Ein kleinerer Anteil besaß jedoch einen zentralen Gedächtnis-(CM)-Phänotyp, welcher bekannt ist für eine Langzeitpersistenz und eine erfolgreiche Etablierung eines T-Zell-Gedächtnis-Pools. Darüber hinaus wurden keine Vaccinia-spezifischen T-Zellen der pockengeimpften Spender expandiert. Wodurch ist die Immunogenität der CMV-Antigene nicht beeinträchtigt ist. Die drei untersuchten MVA-CMV-Vakzinkandidaten erfüllen alle Stabilitäts-, Immunogenitäts- und Sicherheitsbestimmungen der Europäischen Arzneimittelbehörde (EMEA) für virale Vektorimpfstoffe und sind deshalb bereit für die cGMP-Produktion und anschließende klinische Prüfung.
B-Zellen spielen eine wichtige Rolle in der Pathogenese der Rheumatoiden Arthritis. Seit dem Jahr 2006 ist der Anti-CD20-Antikörper Rituximab, welcher eine passagere B-Zell-Depletion induziert, zur Therapie der Rheumatoiden Arthritis zugelassen. In dieser Arbeit wurde das variable Kappa- und Lambda-Leichtkettenrepertoire der CD19+CD27+ B-Gedächtniszellen bei einer Patientin mit aktiver Rheumatoider Arthritis vor und nach B-Zell-Depletion durch Rituximab verglichen. Hierzu wurden nach der Einzelzellsortierung von mononuklären Zellen des peripheren Blutes die Rearrangements des Kappa- und Lambda-Leichtkettenrepertoires amplifiziert, sequenziert und analysiert. Die gefundenen Daten sprechen für die Neubildung eines diversen, polyklonalen und hochmutierten Kappa- und Lambda-Leichtkettenrepertoires. Somit ist davon auszugehen, dass nach der CD20+ B-Zell-Depletion ein funktionsfähiges Repertoire entsteht, welches keine Restriktion für die Infektabwehr aufweist.
Background: Acute myeloid leukemia (AML) is both more common and with more biologically aggressive phenotype in the elderly. Allogenic stem cell transplantation (allo-SCT) is the best treatment option in fit patients. Either HLA-matched unrelated donor (MUD) or haploidentical (Haplo) donor are possible alternative for patients in need. Methods: We retrospectively compared non-T-cell-depleted Haplo (n = 250) to 10/10 MUD (n = 2589) in AML patients >= 60 years. Results: Median follow-up was 23 months. Disease status at transplant differs significantly between the two groups (p < 10(-4)). Reduced intensity conditioning (RIC) was administrated to 73 and 77% of Haplo and MUD, respectively (p = 0.23). Stem cell source was the bone marrow (BM) in 52% of the Haplo and 6% of MUD (p < 10(-4)). Anti-thymocyte globulin (ATG) was most frequently used in MUD (p < 10(-4)) while post-Tx cyclophosphamide (PT-Cy) was given in 62% of Haplo. Engraftment was achieved in 90% of the Haplo vs 97% of MUD (p < 10(-4)). In multivariate analysis, no significant difference was found between Haplo and MUD for acute (a) graft versus host disease (GVHD) grade II-IV, relapse incidence (RI), non-relapse mortality (NRM), leukemia free survival (LFS), graft-versus-host-free-relapse free survival (GRFS), and overall survival (OS). Extensive chronic (c) GVHD was significantly higher for MUD as compared to Haplo (HR 2, p = 0.01, 95% CI 1.17-3.47). A propensity score analysis confirmed the higher risk of extensive cGVHD for MUD without differences for other outcomes. Conclusions: Allo-SCT from both Haplo and MUD are valid option for AML patients >= 60 years of age with similar results. Transplantation from MUD was associated with higher extensive cGVHD. Our findings suggest that Haplo is a suitable and attractive graft source for patients >= 60 with AML in need of allo-SCT.
Background: Chimeric antigen receptor (CAR) T-cells are changing the therapeutic landscape of hematologic malignancies. Severe side effects include cytokine release syndrome (CRS) and immune effector cell-associated neurotoxicity syndrome (ICANS), but prolonged cytopenia has also been reported. The underlying mechanism for prolonged cytopenia is poorly understood so far. Cases: Severe pancytopenia with grade 2-3 anemia was marked 2–3 months after treatment. Laboratory evaluation revealed undetectable levels of haptoglobin with increased reticulocyte counts. Coomb's tests were negative, no schistocytes were detected on blood smear, and infectious causes were ruled out. Increased erythropoiesis without lymphoma infiltration was noted on bone marrow biopsy. A spontaneous increase in haptoglobin and hemoglobin levels was observed after several weeks. For one patient, peripheral CAR-T levels were monitored over time. We observed a decline at the same time as hemoglobin levels began to rise, implying a potential causality. Conclusion: To our knowledge, we describe the first two cases of Coombs-negative hemolytic anemia after CAR-T treatment for B-cell lymphoma. We encourage routine monitoring for hemolytic anemia after CAR-T treatment and also encourage further investigations on the underlying mechanism.
Background
Over the past two decades, there has been a rising trend in malignant melanoma incidence worldwide. In 2008, Germany introduced a nationwide skin cancer screening program starting at age 35. The aims of this study were to analyse the distribution of malignant melanoma tumour stages over time, as well as demographic and regional differences in stage distribution and survival of melanoma patients.
Methods
Pooled data from 61 895 malignant melanoma patients diagnosed between 2002 and 2011 and documented in 28 German population-based and hospital-based clinical cancer registries were analysed using descriptive methods, joinpoint regression, logistic regression and relative survival.
Results
The number of annually documented cases increased by 53.2% between 2002 (N = 4 779) and 2011 (N = 7 320). There was a statistically significant continuous positive trend in the proportion of stage UICC I cases diagnosed between 2002 and 2011, compared to a negative trend for stage UICC II. No trends were found for stages UICC III and IV respectively. Age (OR 0.97, 95% CI 0.97–0.97), sex (OR 1.18, 95% CI 1.11–1.25), date of diagnosis (OR 1.05, 95% CI 1.04–1.06), ‘diagnosis during screening’ (OR 3.24, 95% CI 2.50–4.19) and place of residence (OR 1.23, 95% CI 1.16–1.30) had a statistically significant influence on the tumour stage at diagnosis. The overall 5-year relative survival for invasive cases was 83.4% (95% CI 82.8–83.9%).
Conclusions
No distinct changes in the distribution of malignant melanoma tumour stages among those aged 35 and older were seen that could be directly attributed to the introduction of skin cancer screening in 2008.

Immune checkpoint blockade therapy is beneficial and even curative for some cancer patients. However, the majority don’t respond to immune therapy. Across different tumor types, pre-existing T cell infiltrates predict response to checkpoint-based immunotherapy. Based on in vitro pharmacological studies, mouse models and analyses of human melanoma patients, we show that the cytokine GDF-15 impairs LFA-1/β2-integrin-mediated adhesion of T cells to activated endothelial cells, which is a pre-requisite of T cell extravasation. In melanoma patients, GDF-15 serum levels strongly correlate with failure of PD-1-based immune checkpoint blockade therapy. Neutralization of GDF-15 improves both T cell trafficking and therapy efficiency in murine tumor models. Thus GDF-15, beside its known role in cancer-related anorexia and cachexia, emerges as a regulator of T cell extravasation into the tumor microenvironment, which provides an even stronger rationale for therapeutic anti-GDF-15 antibody development.
Multiple activities are ascribed to the cytokine tumor necrosis factor (TNF) in health and disease. In particular, TNF was shown to affect carcinogenesis in multiple ways. This cytokine acts via the activation of two cell surface receptors, TNFR1, which is associated with inflammation, and TNFR2, which was shown to cause anti-inflammatory signaling. We assessed the effects of TNF and its two receptors on the progression of pancreatic cancer by in vivo bioluminescence imaging in a syngeneic orthotopic tumor mouse model with Panc02 cells. Mice deficient for TNFR1 were unable to spontaneously reject Panc02 tumors and furthermore displayed enhanced tumor progression. In contrast, a fraction of wild type (37.5%), TNF deficient (12.5%), and TNFR2 deficient mice (22.2%) were able to fully reject the tumor within two weeks. Pancreatic tumors in TNFR1 deficient mice displayed increased vascular density, enhanced infiltration of CD4+ T cells and CD4+ forkhead box P3 (FoxP3)+ regulatory T cells (Treg) but reduced numbers of CD8+ T cells. These alterations were further accompanied by transcriptional upregulation of IL4. Thus, TNF and TNFR1 are required in pancreatic ductal carcinoma to ensure optimal CD8+ T cell-mediated immunosurveillance and tumor rejection. Exogenous systemic administration of human TNF, however, which only interacts with murine TNFR1, accelerated tumor progression. This suggests that TNFR1 has basically the capability in the Panc02 model to trigger pro-and anti-tumoral effects but the spatiotemporal availability of TNF seems to determine finally the overall outcome.
Troponin-I-Freisetzung bei kardiochirurgischen Bypassoperationen mit und ohne Herz-Lungen-Maschine

In dieser Studie wurden der Troponin-I-Verlauf, sowie die Serumwerte von Myoglobin, CK und CK-MB prä- und postoperativ nach einem Bypasseingriff am Herzen untersucht. Die Patienten wurden entweder mit Hilfe der Herz-Lungen-Maschine oder im sogenannten Off-Pump-Verfahren operiert. Bei den Patienten der Off-Pump-Gruppe wurde ebenfalls eine Koronarbypassoperation durchgeführt, jedoch am schlagenden Herzen ohne den Einsatz einer Herz-Lungen-Maschine. Die Ergebnisse dieser Studie zeigten, dass es deutliche Unterschiede in den postoperativen Troponin-I-Verläufen zwischen Koronarbypassoperationen mit Herz-Lungen-Maschine und Bypassoperationen am schlagenden Herzen ohne Herz-Lungen-Maschine (Off-Pump-Methode) gibt. Der Troponin-I-Verlauf zeigte in der HLM-Gruppe einen deutlich höheren Troponin-I-Anstieg im gesamten untersuchten postoperativen Verlauf im Vergleich zu den Konzentrationsverläufen des Troponin-I in der Off-Pump-Gruppe. Die weiteren untersuchten Parameter CK, CK-MB und Myoglobin zeigten diese deutlichen Unterschiede in den Konzentrationsverläufen zwischen den beiden Gruppen, wahrscheinlich aufgrund des Gewebeschadens durch die Operation z.B. durch Muskelverletzung, Mediastinaleröffnung, Perikarderöffnung, nicht. In Bezug auf den prozentualen CK-MB-Anteil der beiden Gruppen lag sogar ein annähernd deckungsgleicher Verlauf vor.
Evidence based clinical guidelines are implemented to treat patients efficiently that include efficacy, tolerability but also health economic considerations. This is of particular relevance to the new direct acting antiviral agents that have revolutionized treatment of chronic hepatitis C. For hepatitis C genotypes 2/3 interferon free treatment is already available with sofosbuvir plus ribavirin. However, treatment with sofosbuvir-based regimens is 10-20 times more expensive compared to pegylated interferon alfa and ribavirin (PegIFN/RBV). It has to be discussed if PegIFN/RBV is still an option for easy to treat patients. We assessed the treatment of patients with chronic hepatitis C genotypes 2/3 with PegIFN/RBV in a real world setting according to the latest German guidelines. Overall, 1006 patients were recruited into a prospective patient registry with 959 having started treatment. The intention-to-treat analysis showed poor SVR (GT2 61%, GT3 47%) while patients with adherence had excellent SVR in the per protocol analysis (GT2 96%, GT3 90%). According to guidelines, 283 patients were candidates for shorter treatment duration, namely a treatment of 16 weeks (baseline HCV-RNA <800.000 IU/mL, no cirrhosis and RVR). However, 65% of these easy to treat patients have been treated longer than recommended that resulted in higher costs but not higher SVR rates. In conclusion, treatment with PegIFN/RBV in a real world setting can be highly effective yet similar effective than PegIFN +/- sofosbuvir/RBV in well-selected naive G2/3 patients. Full adherence to guidelines could be further improved, because it would be important in the new era with DAA, especially to safe resources.
Invasive fungal infections are a main cause of morbidity and mortality in cancer patients undergoing intensive chemotherapy regimens. Early antifungal treatment is mandatory to improve survival. Today, a number of effective and better-tolerated but more expensive antifungal agents compared to the former gold standard amphotericin B deoxycholate are available. Clinical decision-making must consider results from numerous studies and published guidelines, as well as licensing status and cost pressure. New developments in antifungal prophylaxis improving survival rates result in a continuous need for actualization. The treatment options for invasive Candida infections include fluconazole, voriconazole, and amphotericin B and its lipid formulations, as well as echinocandins. Voriconazole, amphotericin B, amphotericin B lipid formulations, caspofungin, itraconazole, and posaconazole are available for the treatment of invasive aspergillosis. Additional procedures, such as surgical interventions, immunoregulatory therapy, and granulocyte transfusions, have to be considered. The Infectious Diseases Working Party of the German Society of Hematology and Oncology here presents its 2008 recommendations discussing the dos and do-nots, as well as the problems and possible solutions, of evidence criteria selection.
Although sofosbuvir has been approved for patients with genotypes 2/3 (G2/3), many parts of the world still consider pegylated Interferon alpha (P) and ribavirin (R) as standard of care for G2/3. Patients with rapid virological response (RVR) show response rates >80%. However, SVR (sustained virological response) in non-RVR patients is not satisfactory. Longer treatment duration may be required but evidence from prospective trials are lacking. A total of 1006 chronic HCV genotype 2/3 patients treated with P/R were recruited into a German HepNet multicenter screening registry. Of those, only 226 patients were still HCV RNA positive at week 4 (non-RVR). Non-RVR patients with ongoing response after 24 weeks P-2b/R qualified for OPTEX, a randomized trial investigating treatment extension of additional 24 weeks (total 48 weeks, Group A) or additional 12 weeks (total 36 weeks, group B) of 1.5 \(\mu\)g/kg P-2b and 800-1400 mg R. Due to the low number of patients without RVR, the number of 150 anticipated study patients was not met and only 99 non-RVR patients (n=50 Group A, n=49 Group B) could be enrolled into the OPTEX trial. Baseline factors did not differ between groups. Sixteen patients had G2 and 83 patients G3. Based on the ITT (intention-to-treat) analysis, 68% [55%; 81%] in Group A and 57% [43%; 71%] in Group B achieved SVR (p=0.31). The primary endpoint of better SVR rates in Group A compared to a historical control group (SVR 70%) was not met. In conclusion, approximately 23% of G2/3 patients did not achieve RVR in a real world setting. However, subsequent recruitment in a treatment-extension study was difficult. Prolonged therapy beyond 24 weeks did not result in higher SVR compared to a historical control group.
Die Betazellmasse wird durch Apoptose, Proliferation und Neogenese aus Vorläuferzellen an den Bedarf des Organismus angepasst. Fehlregulationen und Verlust der Anpassungsfähigkeit sind Ursachen für Diabetes mellitus Typ-2. IDX-1 ist sowohl ein Hauptentwicklungsfaktor des embryonalen Pankreas als auch an der Regulation von Neogenese und Proliferation der adulten Betazellen beteiligt. Betazellproliferation und Differenzierung werden durch Faktoren wie GLP-1 oder milde Hyperglykämie stimuliert und gehen mit einer Aktivierung von IDX-1 einher. In der Arbeit sollte der Einfluss von GLP-1 und milder Hyperglykämie auf die Expression, besonders die Transkription, des Transkriptionsfaktors IDX-1 in insulinproduzierenden Betazellen des endokrinen Pankreas untersucht werden. Ferner wurde eine mögliche Autoregulation des IDX-1 Promotors durch IDX-1 untersucht. Als Modell für adulte Betazellen wurden klonale Betazellen INS-1 und MIN6 verwendet. Die IDX-1 Expression wurde auf mRNA Ebene im Northern Blot und auf Proteinebene mittels Western Blot untersucht. Der Promotor des IDX-1 Gens wurde Mithilfe von Luziferasereportergenassays und EMSA untersucht. Die Expression von IDX-1 Protein und mRNA wird durch milde Hyperglykämie stimuliert. Dieser Effekt ist auf eine Aktivierung des IDX-1 Promotors zurückzuführen. Die Aktivierung innerhalb des Promotors konnte auf zwei Regionen eingeschränkt werden. Diese befinden sich im IDX Promotor in den -900 bp bis -300 bp und den 230 bp vor Beginn der kodierenden Sequenz des IDX-1 Gens. Im EMSA konnte ein glukoseabhängiger Komplex (-49 bp bis -44 bp) nachgewiesen werden, an den USF-1 und USF-2 binden. USFs sind für glukoseabhängige Genregulation in Leber und Pankreas bekannt. Eine Mutation der Bindungsstelle führte zum Verlust des Bindungskomplexes. In Luziferasereportergenassays beobachtete man eine Verringerung der glukoseinduzierten Aktivierung. Für GLP-1 konnte kein eindeutiger Einfluss auf die Expression von IDX-1 gezeigt werden. Als Anzeichen für eine mögliche Autoregulation des IDX-1 Promotors durch IDX-1 wurde bei Überexpression von IDX-1 in Betazellen eine verringerte Promotoraktivität festgestellt. Der in dieser Arbeit untersuchte Transkriptionsfaktor IDX-1 spielt eine Schlüsselrolle in der Regulation der Betazellmasse des endokrinen Pankreas. Es ist wichtig die molekularen Mechanismen der Regulation der Betazellmasse zu verstehen; Erkenntnisse darüber eröffnen einerseits ein besseres Verständnis der Pathogenese des Diabetes mellitus, andererseits stellen sie hoffnungsvolle neue Therapieansätze da.
Multiple myeloma remains a largely incurable disease of clonally expanding malignant plasma cells. The bone marrow microenvironment harbors treatment-resistant myeloma cells, which eventually lead to disease relapse in patients. In the bone marrow, CD4\(^{+}\)FoxP3\(^{+}\) regulatory T cells (Tregs) are highly abundant amongst CD4\(^{+}\) T cells providing an immune protective niche for different long-living cell populations, e.g., hematopoietic stem cells. Here, we addressed the functional role of Tregs in multiple myeloma dissemination to bone marrow compartments and disease progression. To investigate the immune regulation of multiple myeloma, we utilized syngeneic immunocompetent murine multiple myeloma models in two different genetic backgrounds. Analyzing the spatial immune architecture of multiple myeloma revealed that the bone marrow Tregs accumulated in the vicinity of malignant plasma cells and displayed an activated phenotype. In vivo Treg depletion prevented multiple myeloma dissemination in both models. Importantly, short-term in vivo depletion of Tregs in mice with established multiple myeloma evoked a potent CD8 T cell- and NK cell-mediated immune response resulting in complete and stable remission. Conclusively, this preclinical in-vivo study suggests that Tregs are an attractive target for the treatment of multiple myeloma.
Chemotherapy, the standard treatment for pancreatic ductal adenocarcinoma (PDAC), has only a modest effect on the outcome of patients with late-stage disease. Investigations of the genetic features of PDAC have demonstrated a frequent occurrence of mutations in genes involved in homologous recombination (HR), especially in the breast cancer susceptibility gene 2 (BRCA2). Olaparib, a poly(ADP-ribose) polymerase (PARP) inhibitor, is approved as a maintenance treatment for patients with advanced PDAC with germline BRCA1/2 mutations following a platinum-containing first-line regimen. Limitations to the use of PARP inhibitors are represented by the relatively small proportion of patients with mutations in BRCA1/2 genes and the modest capability of these substances of inducing objective response. We have previously shown that pancreatic cancer with BRCA2 mutations exhibits a remarkably enhanced sensitivity towards tumor-necrosis-factor-related apoptosis-inducing ligand (TRAIL) receptor-stimulating agents. We thus aimed to investigate the effect of combined treatment with PARP inhibitors and TRAIL receptor-stimulating agents in pancreatic cancer and its dependency on the BRCA2 gene status. The respective effects of TRAIL-targeting agents and the PARP inhibitor olaparib or of their combination were assessed in pancreatic cancer cell lines and patient-derived organoids. In addition, BRCA2-knockout and -complementation models were investigated. The effects of these agents on apoptosis, DNA damage, cell cycle, and receptor surface expression were assessed by immunofluorescence, Western blot, and flow cytometry. PARP inhibition and TRAIL synergized to cause cell death in pancreatic cancer cell lines and PDAC organoids. This effect proved independent of BRCA2 gene status in three independent models. Olaparib and TRAIL in combination caused a detectable increase in DNA damage and a concentration-dependent cell cycle arrest in the G2/M and S cell cycle phases. Olaparib also significantly increased the proportion of membrane-bound death receptor 5. Our results provide a preclinical rationale for the combination of PARP inhibitors and TRAIL receptor agonists for the treatment of pancreatic cancer and suggest that the use of PARP inhibitors could be extended to patients without BRCA2 mutations if used in combination with TRAIL agonists.
This non-interventional, prospective phase IV trial evaluated trabectedin in patients with soft tissue sarcoma (STS) in real-life clinical practice across Germany. The primary endpoints were progression-free survival (PFS) rates at 3 and 6 months, as defined by investigators. Overall, 128 patients from 19 German sites were evaluated for efficacy and 130 for safety. Median age was 58.5 years (range: 23–84) and leiomyosarcoma was the most frequent histotype (n = 45; 35.2%). Trabectedin was mostly used as second/third-line treatment (n = 91; 71.1%). Median PFS was 5.2 months (95% CI: 3.3–6.7), with 60.7% and 44.5% of patients free from progression at 3 and 6 months, respectively. Median overall survival was 15.2 months (95% CI: 9.6–21.4). One patient achieved a complete and 14 patients a partial response, conferring an objective response rate of 11.7%. Decreases in white blood cells (27.0% of patients), platelets (16.2%) and neutrophils (13.1%) and increased alanine aminotransferase (10.8%) were the most common trabectedin-related grade 3/4 adverse drug reactions. Two deaths due to pneumonia and sepsis were considered trabectedin-related. Trabectedin confers clinically meaningful activity in patients with multiple STS histotypes, comparable to that previously observed in clinical trials and other non-interventional studies, and with a manageable safety profile.
In the last few years, monoclonal antibodies (mAbs) such as elotuzumab and daratutumab have brought the treatment of multiple myeloma (MM) into the new era of immunotherapy. More recently, chimeric antigen receptor (CAR) modified T cell, a novel cellular immunotherapy, has been developed for treatment of relapsed/refractory (RR) MM, and early phase clinical trials have shown promising efficacy of CAR T cell therapy. Many patients with end stage RRMM regard CAR T cell therapy as their “last chance” and a “hope of cure”. However, severe adverse events (AEs) and even toxic death related to CAR T cell therapy have been observed. The management of AEs related to CAR T cell therapy represents a new challenge, as the pathophysiology is not fully understood and there is still no well-established standard of management. With regard to CAR T cell associated toxicities in MM, in this review, we will provide an overview of experience from clinical trials, pathophysiology, and management strategies.
TNFR1 and TNFR2 regulate the extrinsic apoptotic pathway in myeloma cells by multiple mechanisms

The huge majority of myeloma cell lines express TNFR2 while a substantial subset of them failed to show TNFR1 expression. Stimulation of TNFR1 in the TNFR1-expressing subset of MM cell lines had no or only a very mild effect on cellular viability. Surprisingly, however, TNF stimulation enhanced cell death induction by CD95L and attenuated the apoptotic effect of TRAIL. The contrasting regulation of TRAIL- and CD95L-induced cell death by TNF could be traced back to the concomitant NFjBmediated upregulation of CD95 and the antiapoptotic FLIP protein. It appeared that CD95 induction, due to its strength, overcompensated a rather moderate upregulation of FLIP so that the net effect of TNF-induced NFjB activation in the context of CD95 signaling is pro-apoptotic. TRAIL-induced cell death, however, was antagonized in response to TNF because in this context only the induction of FLIP is relevant. Stimulation of TNFR2 in myeloma cells leads to TRAF2 depletion. In line with this, we observed cell death induction in TNFR1-TNFR2-costimulated JJN3 cells. Our studies revealed that the TNF-TNF receptor system adjusts the responsiveness of the extrinsic apoptotic pathway in myeloma cells by multiple mechanisms that generate a highly context-dependent net effect on myeloma cell survival.
Tumor necrosis factor (TNF) receptor associated factor-2 (TRAF2) has been originally identified as a protein interacting with TNF receptor 2 (TNFR2) but also binds to several other receptors of the TNF receptor superfamily (TNFRSF). TRAF2, often in concert with other members of the TRAF protein family, is involved in the activation of the classical NFκB pathway and the stimulation of various mitogen-activated protein (MAP) kinase cascades by TNFRSF receptors (TNFRs), but is also required to inhibit the alternative NFκB pathway. TRAF2 has also been implicated in endoplasmic reticulum (ER) stress signaling, the regulation of autophagy, and the control of cell death programs. TRAF2 fulfills its functions by acting as a scaffold, bringing together the E3 ligase cellular inhibitor of apoptosis-1 (cIAP1) and cIAP2 with their substrates and various regulatory proteins, e.g., deubiquitinases. Furthermore, TRAF2 can act as an E3 ligase by help of its N-terminal really interesting new gene (RING) domain. The finding that TRAF2 (but also several other members of the TRAF family) interacts with the latent membrane protein 1 (LMP1) oncogene of the Epstein–Barr virus (EBV) indicated early on that TRAF2 could play a role in the oncogenesis of B-cell malignancies and EBV-associated non-keratinizing nasopharyngeal carcinoma (NPC). TRAF2 can also act as an oncogene in solid tumors, e.g., in colon cancer by promoting Wnt/β-catenin signaling. Moreover, tumor cell-expressed TRAF2 has been identified as a major factor-limiting cancer cell killing by cytotoxic T-cells after immune checkpoint blockade. However, TRAF2 can also be context-dependent as a tumor suppressor, presumably by virtue of its inhibitory effect on the alternative NFκB pathway. For example, inactivating mutations of TRAF2 have been associated with tumor development, e.g., in multiple myeloma and mantle cell lymphoma. In this review, we summarize the various TRAF2-related signaling pathways and their relevance for the oncogenic and tumor suppressive activities of TRAF2. Particularly, we discuss currently emerging concepts to target TRAF2 for therapeutic purposes.
In mammals, megakaryocytes (MKs) in the bone marrow (BM) produce blood platelets, required for hemostasis and thrombosis. MKs originate from hematopoietic stem cells and are thought to migrate from an endosteal niche towards the vascular sinusoids during their maturation. Through imaging of MKs in the intact BM, here we show that MKs can be found within the entire BM, without a bias towards bone-distant regions. By combining in vivo two-photon microscopy and in situ light-sheet fluorescence microscopy with computational simulations, we reveal surprisingly slow MK migration, limited intervascular space, and a vessel-biased MK pool. These data challenge the current thrombopoiesis model of MK migration and support a modified model, where MKs at sinusoids are replenished by sinusoidal precursors rather than cells from a distant periostic niche. As MKs do not need to migrate to reach the vessel, therapies to increase MK numbers might be sufficient to raise platelet counts.
In dieser Arbeit wird aufgezeigt, wie die Versorgungssituation und die damit einhergehende Prognose an einem zertifizierten Pankreaskarzinomzentrum (DKG) aussieht. Betrachtet wurden die Therapieverläufe von 201 Patienten, die im Zeitraum von 01.01.2010 bis 31.12.2016 am Leopoldina Krankenhaus in Schweinfurt sowie dem zugehörigem MVZ aufgrund der Erstdiagnose eines Adenokarzinom des Pankreas behandelt wurden. Das aus den gesammelten Daten errechnete mediane Gesamtüberleben aller Patientin lag bei 8.6 Monaten. Sowohl durch eine Operation mit anschließender adjuvanter Therapie als auch durch eine primär palliative Chemotherapie kann eine signifikante Verlängerung der Überlebenszeit erzielt werden, die immer den möglichen Nebenwirkungen einer zytostatischen Therapie sowie perioperative Risiken gegenübergestellt werden müssen. Die häufig frühen Rezidive nach initial kurativer Operation geben Hinweise, dass es sich beim Adenokarzinom des Pankreas primär um Systemerkrankungen handelt.
Weltweit sind etwa 170 Millionen Menschen chronisch mit Hepatits-C-Virus (HCV) infiziert, in der Bundesrepublik Deutschland leben ca. 500.000 HCV-Infizierte. Ein großes Problem bereitet die hohe Chronifizierungsrate der Erkrankung mit 60 – 80 %. Die Therapieerfolge konnten deutlich gesteigert werden, seitdem die Behandlung aus Interferon in Kombination mit Ribavirin besteht. Es gibt jedoch noch immer viele Patienten, die unzureichend auf die Therapie ansprechen. Es ist Gegenstand der For-schung, die Behandlung der chronischen Hepatitis C weiter zu optimieren. In der vorliegenden Arbeit wurden 622 Ribavirin-Plasmaspiegel von 155 konsekutiven Patienten mit Hepatitis C unter antiviraler Therapie retrospektiv ausgewertet. Ziel dieser Arbeit war es, ein Therapeutisches Drug Monitoring (TDM) für RBV zu evaluieren, bei dem die Dosierung von RBV auf der Basis von RBV-Plasmakonzentrationen angepasst wird. Der Durchschnitt der RBV-Spiegel im Steady State betrug 1980 ng / ml bei großer interindividueller Variabiliät. Einige Patienten wiesen große intraindividuelle Schwan-kungen der RBV-Spiegel auf. Andere Patienten zeigten stabile Spiegel. Als Einflussfak-toren für hohe RBV-Spiegel konnten weibliches Geschlecht und höheres Alter identifiziert werden. Die Spiegel lagen bei den weiblichen Patienten im Durchschnitt bei 2192 ng / ml, bei den männlichen Patienten bei 1904 ng / ml. Die Patienten im Alter von 50 – 72 Jahre hatten RBV-Werte von durchschnittlich 2254 ng / ml, die Patienten im Alter von 21 – 49 Jahre 1933 ng / ml. Ein lediglich sehr geringer Zusammenhang bestand zwischen RBV-Spiegeln und dem Körpergewicht (KG) und zwischen RBV-Spiegeln und der RBV-Dosierung pro KG. Keinen Einfluss zeigte der Body Mass Index (BMI) und der Genotyp. Die Erfolgsrate der Therapie betrug insgesamt 58 %, für den Genotyp 1 und 4 45 %, für den Genotyp 2 und 3 80 %. Bei RBV-Spiegeln größer als 2000 ng / ml in den ersten Therapiewochen steigerte sich die Erfolgsrate auf 73 %. Bei RBV-Spiegeln unter 1000 ng / ml in den ersten Wochen sank die Zahl der Therapieerfolge auf 38 %. Bei den RBV-Mittelwerten des Steady States ab der fünften Therapiewoche bis zum Ende der Therapie konnten hohe oder niedrige RBV-Spiegel nur geringe Änderungen der Er-folgsraten bewirken. Mehr männliche Patienten konnten einen Therapieerfolg erreichen. Die Patienten mit Therapieerfolg waren durchschnittlich jünger. In BMI, KG und RBV-Dosierung unter-schieden sich die Patienten mit oder ohne Therapieerfolg nicht. Die häufigste Nebenwirkung der Therapie mit RBV ist die Anämie. Der durchschnittli-che Hb-Abfall der Patienten betrug 2,6 g / dl. Stärkere Hb-Abfälle waren mit höheren RBV-Spiegeln verbunden. Auch Einfluss auf die Hb-Werte nahmen der Ausgans-Hb-Wert und das Körpergewicht. Geringeren Einfluss hatte das Alter des Patienten und sehr geringen Einfluss die RBV-Dosierung pro KG. 15 Patienten waren mit HIV koinfiziert. Von diesen Patienten wurde bei sechs gleich-zeitig eine antiretrovirale Therapie durchgeführt. Die RBV-Spiegel der HIV-Koinfizierten waren niedriger, im Durchschnitt 1625 ng / ml. Der Therapieerfolg betrug 25 %, der Erfolg der Patienten mit gleichzeitiger HAART 17 %. Die Leberwerte der HIV-positiven Patienten sanken während der Therapie nicht so stark wie bei den HIV-negativen Patienten. Die Hb-Werte während der Therapie waren bei den HIV-positiven Patienten geringfügig niedriger, die Kreatinin-Werte geringfügig höher als bei den HIV-negativen Patienten. Da Therapieerfolg und Nebenwirkungen nicht so stark mit der RBV-Dosierung zusam-menhängen wie mit den RBV-Spiegeln, scheint ein TDM für RBV durchaus sinnvoll, insbesondere in den ersten Therapiemonaten. Gerade die ersten Therapiewochen und Monate sind sowohl für den Therapieerfolg als auch die Anämie entscheidend. Da HIV-koinfizierte Patienten mehr Nebenwirkungen erleiden und es Hinweise auf Medikamenteninteraktionen zwischen RBV und den HIV-Therapeutika gibt, ist die RBV-Plasmaspiegelmessung gerade bei diesem Kollektiv sehr wichtig. Ein TDM für RBV bietet zusammenfassend folgende Vorteile: Durch ein TDM in den ersten Therapiewochen kann die Therapieeffizienz durch schnell erreichte hohe RBV-Spiegel gesteigert werden. Für bestimmte Patientenkollektive bietet es folgende Vorteile: Bei Patienten mit einem Risiko für niedrige RBV-Spiegel kann die The-rapieeffizienz gesichert werden. Bei Patienten mit einem Risiko für hohe RBV-Spiegel kann die Sicher-heit der Therapie gesteigert werden. Die Therapie bei HIV / HCV-koinifizierten Patienten kann optimiert werden. Insbesondere Patienten mit gleichzeitiger antiretroviraler Thera-pie profitieren von größerer Therapiesicherheit.
64 HIV-Patienten unter einer EFV-Therapie, darunter 30 Patienten im Stadium B2 und 20 im Stadium C3 wurden über bis zu 4 Jahre beobachtet. Ca. 80% aller EFV-Spiegel lagen während dieser Beobachtungszeit innerhalb des anzustrebenden Konzentrationsbereiches von 1000 - 4000 ng/ml. Bei den gemessenen EFV-Spiegeln fiel eine erhebliche inter- und intraindividuelle Schwankungsbreite auf. EFV-Spiegel im Serum waren tendenziell nicht signifikant niedriger als EFV-Spiegel im Plasma. Patienten mit einem Alter ≥40 Jahre zeigten tendenziell nicht signifikant höhere EFV-Spiegel als jüngere Patienten. Das Patientengewicht konnte als stärkster Einflussfaktor auf die interindividuelle Streuung der Efavirenzspiegel im Plasma identifiziert werden. Hohe EFV-Spiegel bei Patienten mit begleitender Lebererkrankung, die EFV zusammen mit PI´s (insbesondere Ritonavir) einnehmen oder bei Patienten mit einem unterdurchschnittlichen Körpergewicht (insbesondere Frauen) sind Indikationen für eine Dosisanpassung. Es gab keine signifikante Korrelation zwischen Grad der Therapieadhärenz und den EFV-Spiegeln. Die intraindividuelle Streuung der EFV-Spiegel war bei Patienten mit eingeschränkter Therapieadhärenz signifikant größer als bei Patienten mit guter Therapieadhärenz. Es lies sich ein Trend zu einer niedrigeren EFV-Konzentration im Verlauf nachweisen, der aber nicht statistisch signifikant war. NRTI´s nehmen keinen Einfluß auf EFV-Spiegel. Für Begleitmedikamente wie z.B. Methadon, Cotrimoxazol, Omeprazol und Statine ließ sich keine Wechselwirkung mit EFV nachweisen. Fluconazol zeigte einen leicht senkenden Einfluß auf die EFV-Spiegel. Ein Zusammenhang zwischen EFV-Plasmaspiegeln und Lebererkrankung war im Rahmen dieser Arbeit nicht nachweisbar. Raucher zeigten keine signifikanten Unterschiede in der Verteilung der erzielten EFV-Plasmaspiegel gegenüber nicht rauchenden Patienten. Interaktionen zwischen EFV und PI´s wurden in Bezug auf die erzielten PI-Spiegel nachgewiesen. Hepatitis B- bzw. C- Koinfektion sowie alkoholtoxische Leberzirrhose haben Einfluss auf eine EFV-bedingte Leberwerterhöhung. Eine anhaltende Suppression der Viruslast und ein Anstieg der CD4-Zellzahl konnten nachgewiesen werden. Ein EFV-Spiegel <1000ng/ml hat einen hohen prädiktiven Wert bezüglich eines Therapieversagens. Frauen zeigten im Vergleich zu Männern ein 2-fach erhöhtes Risiko akute Nebenwirkungen zu entwickeln. Ein häufigeres Auftreten bei Patienten mit hoher CD4-Zahl am Therapiebeginn kam innerhalb unseren Kollektivs nicht vor. Das Therapeutische Drug Monitoring ist somit integraler Bestandteil der antiretroviralen Therapie, insbesondere bei chronischen Lebererkrankungen.
Memory B cells have known to play an important role in the pathogenesis of rheumatoid arthritis (RA). With the emergence of B cell-targeted therapies, the modulation of memory B cells appears to be a key therapeutic target. Human peripheral memory B cells can be distinguished based on the phenotypic expression of CD27 and IgD, characterizing the three major B cell subpopulations: CD27+IgD+ pre-switch, CD27+IgD- post-switch, and CD27-IgD- double-negative memory B cells. We evaluated different memory cell populations for activation markers (CD95 and Ki-67) and chemokine receptors (CXCR3 and 4) expressing B cells in active RA, as well as under IL6-R blockade by tocilizumab (TCZ) and TNF-α blockade by adalimumab (ADA). Memory B cells were phenotypically analyzed from RA patients at baseline, week 12, and week 24 under TCZ or ADA treatment, respectively. Using flow cytometry, surface expression of CD95, intracellular Ki-67, and surface expressions of CXCR3 and CXCR4 were determined. Compared with healthy donors (n = 40), the phenotypic analysis of RA patients (n = 80) demonstrated that all three types of memory B cells were activated in RA patients. Surface and intracellular staining of B cells showed a significantly higher percentage of CD95+ (p < 0.0001) and Ki-67+ (p < 0.0001) cells, with numerically altered CXCR3+ and CXCR4+ cells in RA. CD95 and Ki-67 expressions were highest in post-switch memory B cells, whereas CD19+CXCR3+ and CD19+CXCR4+ expressing cells were substantially higher in the pre-switch compartment. In all subsets of the memory B cells, in vivo IL-6R, and TNF-α blockade significantly reduced the enhanced expressions of CD95 and Ki-67. Based on our findings, we conclude that the three major peripheral memory B cell populations, pre-, post-switch, and double-negative B cells, are activated in RA, demonstrating enhanced CD95 and Ki-67 expressions, and varied expression of CXCR3 and CXCR4 chemokine receptors when compared with healthy individuals. This activation can be efficaciously modulated under cytokine inhibition in vivo.
The transcription factor NRF2 is the major mediator of oxidative stress responses and is closely connected to therapy resistance in tumors harboring activating mutations in the NRF2 pathway. In melanoma, such mutations are rare, and it is unclear to what extent melanomas rely on NRF2. Here we show that NRF2 suppresses the activity of the melanocyte lineage marker MITF in melanoma, thereby reducing the expression of pigmentation markers. Intriguingly, we furthermore identified NRF2 as key regulator of immune-modulating genes, linking oxidative stress with the induction of cyclooxygenase 2 (COX2) in an ATF4-dependent manner. COX2 is critical for the secretion of prostaglandin E2 and was strongly induced by H\(_2\)O\(_2\) or TNFα only in presence of NRF2. Induction of MITF and depletion of COX2 and PGE2 were also observed in NRF2-deleted melanoma cells in vivo. Furthermore, genes corresponding to the innate immune response such as RSAD2 and IFIH1 were strongly elevated in absence of NRF2 and coincided with immune evasion parameters in human melanoma datasets. Even in vitro, NRF2 activation or prostaglandin E2 supplementation blunted the induction of the innate immune response in melanoma cells. Transcriptome analyses from lung adenocarcinomas indicate that the observed link between NRF2 and the innate immune response is not restricted to melanoma.
More than 25 years after the first peptide receptor radionuclide therapy (PRRT), the concept of somatostatin receptor (SSTR)-directed imaging and therapy for neuroendocrine tumors (NET) is seeing rapidly increasing use. To maximize the full potential of its theranostic promise, efforts in recent years have expanded recommendations in current guidelines and included the evaluation of novel theranostic radiotracers for imaging and treatment of NET. Moreover, the introduction of standardized reporting framework systems may harmonize PET reading, address pitfalls in interpreting SSTR-PET/CT scans and guide the treating physician in selecting PRRT candidates. Notably, the concept of PRRT has also been applied beyond oncology, e.g. for treatment of inflammatory conditions like sarcoidosis. Future perspectives may include the efficacy evaluation of PRRT compared to other common treatment options for NET, novel strategies for closer monitoring of potential side effects, the introduction of novel radiotracers with beneficial pharmacodynamic and kinetic properties or the use of supervised machine learning approaches for outcome prediction. This article reviews how the SSTR-directed theranostic concept is currently applied and also reflects on recent developments that hold promise for the future of theranostics in this context.
No abstract avDendritic cells (DC) are the most important antigen presenting cells and play a pivotal role in host immunity to infectious agents by acting as a bridge between the innate and adaptive immune systems. Monocyte-derived immature DCs (iDC) were infected with viable resting conidia of Aspergillus fumigatus (Af293) for 12 hours at an MOI of 5; cells were sampled every three hours. RNA was extracted from both organisms at each time point and hybridised to microarrays. iDC cell death increased at 6 h in the presence of A. fumigatus which coincided with fungal germ tube emergence; .80% of conidia were associated with iDC. Over the time course A. fumigatus differentially regulated 210 genes, FunCat analysis indicated significant up-regulation of genes involved in fermentation, drug transport, pathogenesis and response to oxidative stress. Genes related to cytotoxicity were differentially regulated but the gliotoxin biosynthesis genes were down regulated over the time course, while Aspf1 was up-regulated at 9 h and 12 h. There was an up-regulation of genes in the subtelomeric regions of the genome as the interaction progressed. The genes up-regulated by iDC in the presence of A. fumigatus indicated that they were producing a pro-inflammatory response which was consistent with previous transcriptome studies of iDC interacting with A. fumigatus germ tubes. This study shows that A. fumigatus adapts to phagocytosis by iDCs by utilising genes that allow it to survive the interaction rather than just up-regulation of specific virulence genes.
Growing evidence points to multiple myeloma (MM) and its stromal microenvironment using several mechanisms to subvert effective immune and anti-tumor responses. Recent advances have uncovered the tumor-stromal cell influence in regulating the immune-microenvironment and have envisioned targeting these suppressive pathways to improve therapeutic outcomes. Nevertheless, some subgroups of patients include those with particularly unfavorable prognoses. Biological stratification can be used to categorize patient-, disease- or therapy-related factors, or alternatively, these biological determinants can be included in a dynamic model that customizes a given treatment to a specific patient. Genetic heterogeneity and current knowledge enforce a systematic and comprehensive bench-to-bedside approach. Given the increasing role of cancer stem cells (CSCs) in better characterizing the pathogenesis of solid and hematological malignancies, disease relapse, and drug resistance, identifying and describing CSCs is of paramount importance in the management of MM. Even though the function of CSCs is well-known in other cancer types, their role in MM remains elusive. With this review, we aim to provide an update on MM homing and resilience in the bone marrow micro milieu. These data are particularly interesting for clinicians facing unmet medical needs while designing novel treatment approaches for MM.
Panobinostat is an oral pan-histone deacetylase inhibitor developed by Novartis. Panobinostat acts via epigenetic modification and inhibition of the aggresome pathway. In August 2015, the European Commission authorized panobinostat for use in combination with bortezomib and dexamethasone for the treatment of relapsed or relapsed and refractory multiple myeloma (MM) in patients who have received aeyen2 prior regimens including bortezomib and an immunomodulatory drug. In January 2016, the National Institute for Health and Care Excellence recommended panobinostat for use in the same combination and patient population. The authorization and recommendation were based on results from the pivotal phase 3 PANORAMA 1 (NCT01023308) clinical trial, which demonstrated an improvement in median progression-free survival of 7.8 months for the three-drug combination compared with placebo plus bortezomib and dexamethasone in this patient population. This review will discuss the current treatment landscape for relapsed/refractory MM, the mechanism of action of panobinostat, clinical data supporting the European authorization, concerns about safety and strategies for mitigating toxicity, and how panobinostat fits into the current MM landscape in Europe.
We assume that a specific health constraint, e.g., a certain aspect of bodily function or quality of life that is measured by a variable X, is absent (or irrelevant) in a healthy reference population (Ref0), and it is materially present and precisely measured in a diseased reference population (Ref1). We further assume that some amount of this constraint of interest is suspected to be present in a population under study (SP). In order to quantify this issue, we propose the introduction of an intuitive measure, the population comparison index (PCI), that relates the mean value of X in population SP to the mean values of X in populations Ref0 and Ref1. This measure is defined as PCI[X] = (mean[X|SP] − mean[X|Ref0])/(mean[X|Ref1] − mean[X|Ref0]) × 100[%], where mean[X|.] is the average value of X in the respective group of individuals. For interpretation, PCI[X] ≈ 0 indicates that the values of X in the population SP are similar to those in population Ref0, and hence, the impairment measured by X is not materially present in the individuals in population SP. On the other hand, PCI[X] ≈ 100 means that the individuals in SP exhibit values of X comparable to those occurring in Ref1, i.e., the constraint of interest is equally present in populations SP and Ref1. A value of 0 < PCI[X] < 100 indicates that a certain percentage of the constraint is present in SP, and it is more than in Ref0 but less than in Ref1. A value of PCI[X] > 100 means that population SP is even more affected by the constraint than population Ref1.
Despite therapeutic advances multiple myeloma remains largely incurable, and novel therapeutic concepts are needed. The Hsp90-chaperone is a reasonable therapeutic target, because it maintains oncogenic signaling of multiple deregulated pathways. However, in contrast to promising pre-clinical results, only limited clinical efficacy has been achieved through pharmacological Hsp90 inhibition. Because Hsp70 has been described to interact functionally with the Hsp90-complex, we analyzed the suitability of Hsp72 and Hsp73 as potential additional target sites. Expression of Hsp72 and Hsp73 in myeloma cells was analyzed by immunohistochemical staining and western blotting. Short interfering RNA-mediated knockdown or pharmacological inhibition of Hsp72 and Hsp73 was performed to evaluate the role of these proteins in myeloma cell survival and for Hsp90-chaperone function. Furthermore, the role of PI3K-dependent signaling in constitutive and inducible Hsp70 expression was investigated using short interfering RNA-mediated and pharmacological PI3K inhibition. Hsp72 and Hsp73 were frequently overexpressed in multiple myeloma. Knockdown of Hsp72 and/or Hsp73 or treatment with VER-155008 induced apoptosis of myeloma cells. Hsp72/Hsp73 inhibition decreased protein levels of Hsp90-chaperone clients affecting multiple oncogenic signaling pathways, and acted synergistically with the Hsp90 inhibitor NVP-AUY922 in the induction of death of myeloma cells. Inhibition of the PI3K/Akt/GSK3b pathway with short interfering RNA or PI103 decreased expression of the heat shock transcription factor 1 and down-regulated constitutive and inducible Hsp70 expression. Treatment of myeloma cells with a combination of NVP-AUY922 and PI103 resulted in additive to synergistic cytotoxicity. In conclusion, Hsp72 and Hsp73 sustain Hsp90-haperone function and critically contribute to the survival of myeloma cells. Translation of Hsp70 inhibition into the clinic is therefore highly desirable. Treatment with PI3K inhibitors might represent an alternative therapeutic strategy to target Hsp70.
Interleukin-27 (IL27) is a type-I cytokine of the IL6/IL12 family and is predominantly secreted by activated macrophages and dendritic cells. We show that IL27 induces STAT factor phosphorylation in cancerous cell lines of different tissue origin. IL27 leads to STAT1 phosphorylation and recapitulates an IFN--like response in the microarray analyses, with up-regulation of genes involved in antiviral defense, antigen presentation, and immune suppression. Like IFN-, IL27 leads to an up-regulation of TAP2 and MHC-I proteins, which mediate increased tumor immune clearance. However, both cytokines also upregulate proteins such as PD-L1 (CD274) and IDO-1, which are associated with immune escape of cancer. Interestingly, differential expression of these genes was observed within the different cell lines and when comparing IL27 to IFN-. In coculture experiments of hepatocellular carcinoma (HCC) cells with peripheral blood mononuclear cells, pre-treatment of the HCC cells with IL27 resulted in lowered IL2 production by anti-CD3/-CD28 activated T-lymphocytes. Addition of anti-PD-L1 antibody, however, restored IL2 secretion. The levels of other T(H)1 cytokines were also enhanced or restored upon administration of anti-PD-L1. In addition, we show that the suppression of IL27 signaling by IL6-type cytokine pre-stimulationmimicking a situation occurring, for example, in IL6-secreting tumors or in tumor inflammation-induced cachexiacan be antagonized by antibodies against IL6-type cytokines or their receptors. Therapeutically, the antitumor effects of IL27 (mediated, e.g., by increased antigen presentation) might thus be increased by combining IL27 with blocking antibodies against PD-L1 or/and IL6-type cytokines.
In contrast to other haematological malignancies, targeted immunotherapy has not entered standard treatment regimens for de novo or relapsed multiple myeloma (MM) yet. While a number of IgG-formatted monoclonal antibodies are currently being evaluated in clinical trials in MM, our study aimed to investigate whether the fully human IgM monoclonal antibody PAT-SM6 that targets a tumour-specific variant of the heat shock protein GRP78 might be an attractive candidate for future immunotherapeutic approaches. We here show that GRP78 is stably and consistently expressed on the surface on tumour cells from patients with de novo, but also relapsed MM and that binding of PAT-SM6 to MM cells can specifically exert cytotoxic effects on malignant plasma cells, whereas non-malignant cells are not targeted. We demonstrate that the induction of apoptosis and, to a lesser extent, complement dependent cytotoxicity is the main mode of action of PAT-SM6, whereas antibody dependent cellular cytotoxicity does not appear to contribute to the cytotoxic properties of this antibody. Given the favourable safety profile of PAT-SM6 in monkeys, but also in a recent phase I trial in patients with malignant melanoma, our results form the basis for a planned phase I study in patients with relapsed MM.
Utilizing 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography (PET)/computed tomography (CT), we performed this pilot study to evaluate the link between cytogenetic/genomic markers and imaging patterns in relapsed/refractory (RR) multiple myeloma (MM). We retrospectively analyzed data of 24 patients with RRMM who were treated at our institution between November 2018 and February 2020. At the last relapse/progression, patients had been treated with a median of three (range 1–10) lines of therapy. Six (25%) patients showed FDG avid extramedullary disease without adjacency to bone. We observed significantly higher maximum standardized uptake values (SUV\(_{max}\)) in patients harboring del(17p) compared with those without del(17p) (p = 0.025). Moreover, a high SUV\(_{max}\) of >15 indicated significantly shortened progression-free survival (PFS) (p = 0.01) and overall survival (OS) (p = 0.0002). One female patient exhibited biallelic TP53 alteration, i.e., deletion and mutation, in whom an extremely high SUV\(_{max}\) of 37.88 was observed. In summary, this pilot study suggested a link between del(17p)/TP53 alteration and high SUV\(_{max}\) on 18F-FDG PET/CT in RRMM patients. Further investigations are highly warranted at this point.
The KISS1 Receptor as an In Vivo Microenvironment Imaging Biomarker of Multiple Myeloma Bone Disease

Multiple myeloma is one of the most common hematological diseases and is characterized by an aberrant proliferation of plasma cells within the bone marrow. As a result of crosstalk between cancer cells and the bone microenvironment, bone homeostasis is disrupted leading to osteolytic lesions and poor prognosis. Current diagnostic strategies for myeloma typically rely on detection of excess monoclonal immunoglobulins or light chains in the urine or serum. However, these strategies fail to localize the sites of malignancies. In this study we sought to identify novel biomarkers of myeloma bone disease which could target the malignant cells and/or the surrounding cells of the tumor microenvironment. From these studies, the KISS1 receptor (KISS1R), a G-protein-coupled receptor known to play a role in the regulation of endocrine functions, was identified as a target gene that was upregulated on mesenchymal stem cells (MSCs) and osteoprogenitor cells (OPCs) when co-cultured with myeloma cells. To determine the potential of this receptor as a biomarker, in vitro and in vivo studies were performed with the KISS1R ligand, kisspeptin, conjugated with a fluorescent dye. In vitro microscopy showed binding of fluorescently-labeled kisspeptin to both myeloma cells as well as MSCs under direct co-culture conditions. Next, conjugated kisspeptin was injected into immune-competent mice containing myeloma bone lesions. Tumor-burdened limbs showed increased peak fluorescence compared to contralateral controls. These data suggest the utility of the KISS1R as a novel biomarker for multiple myeloma, capable of targeting both tumor cells and host cells of the tumor microenvironment.
A disturbance in the symbiotic mutualism between the intestinal microbiome and the human host’s organism (syn. dysbiosis) accompanies the development of a variety of inflammatory and metabolic diseases that comprise the Metabolic Syndrome, chronic inflammatory gut diseases like Crohn’s disease, Non-alcoholic fatty liver disease (NAFLD) and cardiovascular diseases, among others. The changed uptake and effectiveness of short chain fatty acids (SCFAs) as well as an increase of the intestinal permeability are common, interdependent disease elements in this regard. Short chain fatty acids are end-products of intestinal bacterial fermentation and affect the mucosal barrier integrity via numerous molecular mechanisms.
There is evidence to suggest, that SCFAs have a modulating influence on Signal transducer and activator of transcription 3 (STAT3) in intestinal epithelial cells. STAT3 is a central gene-transcription factor in signaling pathways of proliferation and inflammation. It can be activated by growth factors and other intercellular signaling molecules like the cytokine Oncostatin M (OSM). The mode of STAT3’s activation exhibits, finally, a decisive influence on the immunological balance at the intestinal mucosa. Therefore, the posttranslational modification of STAT3 under the influence of SCFAs is likely to be a very important factor within the development and -progression of dysbiosis-associated diseases.
In this study, a clear positive in vitro-effect of the short chain fatty acid butyrate on the posttranslational serine727-phosphorylation of STAT3 and its total protein amount in the human adenocarcinoma cell line CACO2 is verified. Moreover, an increased gene expression of the OSM-receptor subunit OSMRβ can be observed after butyrate incubation. Histone deacetylase inhibition is shown to have a predominant role in these effects. Furthermore, a subsequent p38 MAPK-activation by Butyrate is found to be a key molecular mechanism regarding the STAT3-phosphorylation at serine727-residues. To consider the portion of butyrate receptor signaling in this context in future assays, a CACO-2 cell 3D-culture model is introduced in which an improvement of the GPR109A-receptor expression in CACO-2 cells is accomplished.
Background
Multiple myeloma (MM) is the third most common hematologic malignancy with increasing importance due to improving treatment strategies and long-term outcomes in an aging population. This study aims to analyse influencing factors on health-related quality of life (HRQoL), such as treatment strategies, participation in a clinical trial and patient characteristics like anxiety, depression, gender, and age. A better understanding of the individual factors in context with HRQoL could provide a helpful instrument for clinical decisions.
Methods
In this prospective observational study, the HRQoL of MM patients with different therapies (first-line and relapse) was quantified by standardized questionnaires (EORTC QLQ-C30 and -MY20) in the context of sociodemographic data, individual anxiety and depressiveness (PHQ-4), and a selected number of clinical parameters and symptoms at defined time-points before, during, and after therapy.
Results
In total, 70 patients were included in the study. The median age of the study cohort was 62 years. 44% were female and 56% were male patients. More than half of the patients were fully active with an ECOG 0. Global health status was significantly higher in patients with first-line treatment and even increased after start of therapy, while the pain level decreased. In contrast, patients with relapsed MM reported a decreasing global health status and increasing pain. Additionally, there was a higher global health status in less anxious/depressive patients. HRQoL decreased significantly after start of chemotherapy in the parameters body image, side effects of treatment, and cognitive functioning. Tandem stem-cell transplantation was not found to be a risk factor for higher impairment of HRQoL. Participation in a clinical study led to an improvement of most aspects of HRQoL. Among others, increased anxiety and depression, female gender, older age, impaired performance status, and recurrent disease can be early indicators for a reduced HRQoL.
Conclusion
This study showed the importance of regular longitudinal assessments of patient reported outcomes (PROs) in routine clinical care. For the first time, to our knowledge, we were able to demonstrate a potential impact between participation in clinical trials and HRQoL. However, due to frequently restrictive inclusion criteria for clinical trials, these MM patients might not be directly comparable with patients treated within standard therapy concepts. Further studies are needed to clarify the relevance of this preliminary data in order to develop an individualized, patient-centred, therapy concept.
Myeloma is characterized by extensive inter-patient genomic heterogeneity due to multiple different initiating events. A recent multi-region sequencing study demonstrated spatial differences, with progression events, such as TP53 mutations, frequently being restricted to focal lesions. In this review article, we describe the clinical impact of these two types of tumor heterogeneity. Target mutations are often dominant at one site but absent at other sites, which poses a significant challenge to personalized therapy in myeloma. The same holds true for high-risk subclones, which can be locally restricted, and as such not detectable at the iliac crest, which is the usual sampling site. Imaging can improve current risk classifiers and monitoring of residual disease, but does not allow for deciphering the molecular characteristics of tumor clones. In the era of novel immunotherapies, the clinical impact of heterogeneity certainly needs to be re-defined. Yet, preliminary observations indicate an ongoing impact of spatial heterogeneity on the efficacy of monoclonal antibodies. In conclusion, we recommend combining molecular tests with imaging to improve risk prediction and monitoring of residual disease. Overcoming intra-tumor heterogeneity is the prerequisite for curing myeloma. Novel immunotherapies are promising but research addressing their impact on the spatial clonal architecture is highly warranted.
Prevention of the effectiveness of anti-tumor immune responses is one of the canonical cancer hallmarks. The competition for crucial nutrients within the tumor microenvironment (TME) between cancer cells and immune cells creates a complex interplay characterized by metabolic deprivation. Extensive efforts have recently been made to understand better the dynamic interactions between cancer cells and surrounding immune cells. Paradoxically, both cancer cells and activated T cells are metabolically dependent on glycolysis, even in the presence of oxygen, a metabolic process known as the Warburg effect. The intestinal microbial community delivers various types of small molecules that can potentially augment the functional capabilities of the host immune system. Currently, several studies are trying to explore the complex functional relationship between the metabolites secreted by the human microbiome and anti-tumor immunity. Recently, it has been shown that a diverse array of commensal bacteria synthetizes bioactive molecules that enhance the efficacy of cancer immunotherapy, including immune checkpoint inhibitor (ICI) treatment and adoptive cell therapy with chimeric antigen receptor (CAR) T cells. In this review, we highlight the importance of commensal bacteria, particularly of the gut microbiota-derived metabolites that are capable of shaping metabolic, transcriptional and epigenetic processes within the TME in a therapeutically meaningful way.
Peptide Receptor Radionuclide Therapy (PRRT) for the treatment of neuroendocrine tumors may lead to kidney deterioration. This study aimed to evaluate the suitability of \(^{99m}\)Tc-mercaptoacetyltriglycine (\(^{99m}\)Tc-MAG3) clearance for the early detection of PRRT-induced changes on tubular extraction (TE). TE rate (TER) was measured prior to 128 PRRT cycles (7.6±0.4 GBq \(^{177}\)Lu-octreotate/octreotide each) in 32 patients. TER reduction during PRRT was corrected for age-related decrease and analyzed for the potential to predict loss of glomerular filtration (GF). The GF rate (GFR) as measure for renal function was derived from serum creatinine. The mean TER was 234 ± 53 ml/min/1.73 m² before PRRT (baseline) and 221 ± 45 ml/min/1.73 m² after a median follow-up of 370 days. The age-corrected decrease (mean: -3%, range: -27% to +19%) did not reach significance (p=0.09) but significantly correlated with the baseline TER (Spearman p=-0.62, p<0.001). Patients with low baseline TER showed an improved TER after PRRT, high decreases were only observed in individuals with high baseline TER. Pre-therapeutic TER data were inferior to plasma creatinine-derived GFR estimates in predicting late nephropathy. TER assessed by \(^{99m}\)Tc-MAG3clearance prior to and during PRRT is not suitable as early predictor of renal injury and an increased risk for late nephropathy.
Multiple myeloma (MM) displays an NFκB activity-related gene expression signature and about 20% of primary MM samples harbor genetic alterations conducive to intrinsic NFκB signaling activation. The relevance of blocking the classical versus the alternative NFκB signaling pathway and the molecular execution mechanisms involved, however, are still poorly understood. Here, we comparatively tested NFκB activity abrogation through TPCA-1 (an IKK2 inhibitor), BAY 11-7082 (an IKK inhibitor poorly selective for IKK1 and IKK2), and MLN4924 (an NEDD8 activating enzyme (NAE)-inhibitor), and analyzed their anti-MM activity. Whereas TPCA-1 interfered selectively with activation of the classical NFκB pathway, the other two compounds inhibited classical and alternative NFκB signaling without significant discrimination. Noteworthy, whereas TPCA-1 and MLN4924 elicited rather mild anti-MM effects with slight to moderate cell death induction after 1 day BAY 11-7082 was uniformly highly toxic to MM cell lines and primary MM cells. Treatment with BAY 11-7082 induced rapid cell swelling and its initial effects were blocked by necrostatin-1 or the ROS scavenger BHA, but a lasting protective effect was not achieved even with additional blockade of caspases. Because MLN4924 inhibits the alternative NFκB pathway downstream of IKK1 at the level of p100 processing, the quite discordant effects between MLN4924 and BAY 11-7082 must thus be due to blockade of IKK1-mediated NFκB-independent necrosis-inhibitory functions or represent an off-target effect of BAY 11-7082. In accordance with the latter, we further observed that concomitant knockdown of IKK1 and IKK2 did not have any major short-term adverse effect on the viability of MM cells.
Farnesol, produced by the polymorphic fungus Candida albicans, is the first quorum-sensing molecule discovered in eukaryotes. Its main function is control of C. albicans filamentation, a process closely linked to pathogenesis. In this study, we analyzed the effects of farnesol on innate immune cells known to be important for fungal clearance and protective immunity. Farnesol enhanced the expression of activation markers on monocytes (CD86 and HLA-DR) and neutrophils (CD66b and CD11b) and promoted oxidative burst and the release of proinflammatory cytokines (tumor necrosis factor alpha [TNF-\(\alpha\)] and macrophage inflammatory protein 1 alpha [MIP-1 \(\alpha\)]). However, this activation did not result in enhanced fungal uptake or killing. Furthermore, the differentiation of monocytes to immature dendritic cells (iDC) was significantly affected by farnesol. Several markers important for maturation and antigen presentation like CD1a, CD83, CD86, and CD80 were significantly reduced in the presence of farnesol. Furthermore, farnesol modulated migrational behavior and cytokine release and impaired the ability of DC to induce T cell proliferation. Of major importance was the absence of interleukin 12 (IL-12) induction in iDC generated in the presence of farnesol. Transcriptome analyses revealed a farnesol-induced shift in effector molecule expression and a down-regulation of the granulocyte-macrophage colony-stimulating factor (GM-CSF) receptor during monocytes to iDC differentiation. Taken together, our data unveil the ability of farnesol to act as a virulence factor of C. albicans by influencing innate immune cells to promote inflammation and mitigating the Th1 response, which is essential for fungal clearance.
Pulmonary mucosal immune response is critical for preventing opportunistic Aspergillus fumigatus infections. Although fungus‐specific CD4\(^{+}\) T cells in blood are described to reflect the actual host–pathogen interaction status, little is known about Aspergillus‐specific pulmonary T‐cell responses. Here, we exploit the domestic pig as human‐relevant large animal model and introduce antigen‐specific T‐cell enrichment in pigs to address Aspergillus‐specific T cells in the lung compared to peripheral blood. In healthy, environmentally Aspergillus‐exposed pigs, the fungus‐specific T cells are detectable in blood in similar frequencies as observed in healthy humans and exhibit a Th1 phenotype. Exposing pigs to 10\(^{6}\) cfu/m\(^{3}\) conidia induces a long‐lasting accumulation of Aspergillus‐specific Th1 cells locally in the lung and also systemically. Temporary immunosuppression during Aspergillus‐exposure showed a drastic reduction in the lung‐infiltrating antifungal T‐cell responses more than 2 weeks after abrogation of the suppressive treatment. This was reflected in blood, but to a much lesser extent. In conclusion, by using the human‐relevant large animal model the pig, this study highlights that the blood clearly reflects the mucosal fungal‐specific T‐cell reactivity in environmentally exposed as well as experimentally exposed healthy pigs. But, immunosuppression significantly impacts the mucosal site in contrast to the initial systemic immune response.
Monoclonal gammopathy of undetermined significance is one of the most common pre-malignant disorders. IgG and IgA monoclonal gammopathy of undetermined significance are precursor conditions of multiple myeloma; light-chain monoclonal gammopathy of undetermined significance of light-chain multiple myeloma; and IgM monoclonal gammopathy of undetermined significance of Waldenstrom's macroglobulinemia and other lymphoproliferative disorders. Clonal burden, as determined by bone marrow plasma cell percentage or M-protein level, as well as biological characteristics, including heavy chain isotype and light chain production, are helpful in predicting risk of progression of monoclonal gammopathy of undetermined significance to symptomatic disease. Furthermore, alterations in the bone marrow microenvironment of monoclonal gammopathy of undetermined significance patients result in an increased risk of venous and arterial thrombosis, infections, osteoporosis, and bone fractures. In addition, the small clone may occasionally be responsible for severe organ damage through the production of a monoclonal protein that has autoantibody activity or deposits in tissues. These disorders are rare and often require therapy directed at eradication of the underlying plasma cell or lymphoplasmacytic clone. In this review, we provide an overview of the clinical relevance of monoclonal gammopathy of undetermined significance. We also give general recommendations of how to diagnose and manage patients with monoclonal gammopathy of undetermined significance.
Lenalidomide is an integral, yet evolving, part of current treatment pathways for both transplant-eligible and transplant-ineligible patients with newly diagnosed multiple myeloma (NDMM). It is approved in combination with dexamethasone as first-line therapy for transplant-ineligible patients with NDMM, and as maintenance treatment following autologous stem cell transplantation (ASCT). Although strong clinical trial evidence has supported the integration of lenalidomide into current treatment paradigms for NDMM, applying those paradigms to individual patients and determining which patients are most likely to benefit from lenalidomide treatment are more complex. In this paper, we utilize the available clinical trial evidence to provide recommendations for patient selection and lenalidomide dosing in both the first-line setting in patients ineligible for ASCT and the maintenance setting in patients who have undergone ASCT. In addition, we provide guidance on management of those adverse events that are most commonly associated with lenalidomide treatment, and consider the optimal selection and sequencing of next-line agents following long-term frontline or maintenance treatment with lenalidomide.
Immuno‐oncology therapies engage the immune system to treat cancer. BiTE (bispecific T‐cell engager) technology is a targeted immuno‐oncology platform that connects patients' own T cells to malignant cells. The modular nature of BiTE technology facilitates the generation of molecules against tumor‐specific antigens, allowing off‐the‐shelf immuno‐oncotherapy. Blinatumomab was the first approved canonical BiTE molecule and targets CD19 surface antigens on B cells, making blinatumomab largely independent of genetic alterations or intracellular escape mechanisms. Additional BiTE molecules in development target other hematologic malignancies (eg, multiple myeloma, acute myeloid leukemia, and B‐cell non‐Hodgkin lymphoma) and solid tumors (eg, prostate cancer, glioblastoma, gastric cancer, and small‐cell lung cancer). BiTE molecules with an extended half‐life relative to the canonical BiTE molecules are also being developed. Advances in immuno‐oncology made with BiTE technology could substantially improve the treatment of hematologic and solid tumors and offer enhanced activity in combination with other treatments.
Diese Arbeit befasste sich mit der Analyse geschlechtsspezifischer Besonder-heiten des Metabolismus von Proteaseinhibitoren. Insbesondere wurde auf die Pharmakokinetik des ATV und LPV eingegangen, außerdem wurden die Dosierungen der Medikamente, die Wirksamkeit und die Nebenwirkungen untersucht. Hierzu waren n=152 HIV-positive Patienten eingeschlossen, wovon n=96 Patienten mit LPV therapiert wurden; n=56 nahmen ATV ein. Insgesamt waren n=127 Probanden (83,55%) männlich und n=25 (16,45%) weiblich. Die Studie wurde im Sinne einer retrospektiven Längsschnittuntersuchung durchgeführt. Es ließen sich bei beiden PIs keine Unterschiede der Plasmaspiegel der Männer und Frauen ausmachen. Ebenfalls waren im Langzeitverlauf keine signifikanten Schwankungen der Spiegel beider Geschlechter zu beobachten. Die interindividuelle Schwankung betrug bei der ATV-Beobachtung 22,1% (Männer) und 51,4% (Frauen) und bei der LPV-Studie 13,2% (Männer) und 30,1% (Frauen). Die intraindividuelle Schwankung war höher und ergab beim ATV-Kollektiv 57,7% (Männern) und 39,7% (Frauen) und beim LPV-Arm 42,8% (Männer) und 41,4% (Frauen). Im Vergleich der Geschlechter ließ sich sowohl im ATV- als auch im LPV-Kollektiv bei beiden Geschlechtern die mittlere Dosis pro Kilogramm Körperge-wicht nicht signifikant mit den Plasmaspiegeln korrelieren. In der ATV-Beobachtung erhalten die Frauen signifikant niedrigere Dosierungen (p=0,000*), im Gegensatz zu der LPV-Beobachtung, in der die Frauen signifikant höhere Dosierung (p=0,000*) bekommen. Die Wirksamkeit wurde zum einen durch den Abfall der Viruslast bestimmt. Hier ließ sich ein wesentlicher (p=0,000*) Abfall nach einem Monat bei ATV und LPV beobachten. Zum anderen war der Anstieg der CD4-Zellen im ATV-Kollektiv bei den Männern zu beobachten (p=0,000*). Im LPV-Kollektiv stiegen sie innerhalb eines Monats bei beiden Geschlechtern signifikant (p=0,000*) an. Das Bilirubin stieg bei der ATV-Beobachtung innerhalb eines Monats signifikant (p=0,000*) an und wies bei beiden Geschlechtern eine wesentliche Korrelation mit den Plasmaspiegeln auf, mit einem p-Wert von 0,017* für die Männer und einem p-Wert von 0,000* für die Frauen. Die GOT- und GPT-Studie zeigte hinsichtlich der Langzeitbeobachtung und der Korrelation mit den Spiegeln bei keinem Medikament eine Auffälligkeit. Die Beobachtung der gGT-Werte ergab einen signifikanten Abfall der Werte nach sechs Monaten beim ATV-Kollektiv (0,025*). Das HDL stieg nach drei Monaten bei der ATV-Gruppe signifikant an (p=0,000*), sowie bei der LPV-Gruppe nach einem Monat (p=0,000*). Beim LDL verhielt sich der Anstieg ähnlich, hier gab es beim ATV nach drei Monaten einen p-Wert von 0,008* und beim LPV nach einem Monat einen von 0,033*. Außerdem verhielt sich bei der LPV-Beobachtung die Korrelation der LDL-Werte der Frauen mit den LPV-Spiegeln signifikant (p=0,012*). Ebenfalls ließen sich beim TRG wesentliche Anstiege verzeichnen, so war die-ses Ergebnis beim ATV nach sechs Monaten mit einem p-Wert von 0,030* und beim LPV nach einem Monat mit einem p-Wert von 0,000* zu beweisen. Das Cholesterin stieg bei der LPV-Beobachtung innerhalb eines Monats signifi-kant an (p=0,000) und war auch bei den Frauen wesentlich mit den Plas-maspiegeln zu korrelieren (p=0,013*). Insgesamt waren nur bei drei der oben genannten Kategorien Unterschiede zwischen den Geschlechtern zu beobachten. Dies waren zum einen die CD4-Zellen, denn hier ergab sich bei den Männern eine signifikant höhere Anzahl (p=0,038*). Weiterhin war der Bilirubinwert der Männer höher als der der Frau-en (p=0,000*). Zuletzt wiesen die Männer auch einen höheren TRG-Wert auf (p=0,009*). Schlussfolgernd ist die Aussage möglich, dass sich in dieser Langzeitbeobach-tung geschlechtsspezifische Unterschiede zwischen Männern und Frauen bezüglich der ATV- und LPV-Plasmaspiegel ausschließen lassen. Weiterhin ist auch im Langzeitverlauf kein signifikanter Unterschied zwischen Männern und Frauen im Ansprechen auf die Therapie oder in der Häufigkeit von unerwünschten Ereignissen während der Therapie zu beobachten.
The liver‐derived, circulating transport protein transthyretin (TTR) is the cause of systemic hereditary (ATTRv) and wild‐type (ATTRwt) amyloidosis. TTR stabilization and knockdown are approved therapies to mitigate the otherwise lethal disease course. To date, the variety in phenotypic penetrance is not fully understood. This systematic review summarizes the current literature on TTR pathophysiology with its therapeutic implications. Tetramer dissociation is the rate‐limiting step of amyloidogenesis. Besides destabilizing TTR mutations, other genetic (RBP4, APCS, AR, ATX2, C1q, C3) and external (extracellular matrix, Schwann cell interaction) factors influence the type of onset and organ tropism. The approved small molecule tafamidis stabilizes the tetramer and significantly decelerates the clinical course. By sequence‐specific mRNA knockdown, the approved small interfering RNA (siRNA) patisiran and antisense oligonucleotide (ASO) inotersen both significantly reduce plasma TTR levels and improve neuropathy and quality of life compared to placebo. With enhanced hepatic targeting capabilities, GalNac‐conjugated siRNA and ASOs have recently entered phase III clinical trials. Bivalent TTR stabilizers occupy both binding groves in vitro, but have not been tested in trials so far. Tolcapone is another stabilizer with the potential to cross the blood–brain barrier, but its half‐life is short and liver failure a potential side effect. Amyloid‐directed antibodies and substances like doxycycline aim at reducing the amyloid load, however, none of the yet developed antibodies has successfully passed clinical trials. ATTR‐amyloidosis has become a model disease for pathophysiology‐based treatment. Further understanding of disease mechanisms will help to overcome the remaining limitations, including application burden, side effects, and blood–brain barrier permeability.
Purpose
Multiple myeloma is a hematologic malignancy originating from clonal plasma cells. Despite effective therapies, outcomes are highly variable suggesting marked disease heterogeneity. The role of functional imaging for therapeutic management of myeloma, such as positron emission tomography with 2-deoxy-2-[18F]fluoro-D-glucose (18F-FDG-PET), remains to be determined. Although some studies already suggested a prognostic value of 18F-FDG-PET, more specific tracers addressing hallmarks of myeloma biology, e.g. paraprotein biosynthesis, are needed. This study evaluated the amino acid tracers L-methyl-[11C]-methionine (11C-MET) and [18F]-fluoroethyl-L-tyrosine (18F-Fet) for their potential to image myeloma and to characterize tumor heterogeneity.
Experimental Design
To study the utility of 11C-MET, 18F-Fet and 18F-FDG for myeloma imaging, time activity curves were compared in various human myeloma cell lines (INA-6, MM1.S, OPM-2) and correlated to cell-biological characteristics, such as marker gene expression and immunoglobulin levels. Likewise, patient-derived CD138+ plasma cells were characterized regarding uptake and biomedical features.
Results
Using myeloma cell lines and patient-derived CD138+ plasma cells, we found that the relative uptake of 11C-MET exceeds that of 18F-FDG 1.5- to 5-fold and that of 18F-Fet 7- to 20-fold. Importantly, 11C-MET uptake significantly differed between cell types associated with worse prognosis (e.g. t(4;14) in OPM-2 cells) and indolent ones and correlated with intracellular immunoglobulin light chain and cell surface CD138 and CXCR4 levels. Direct comparison of radiotracer uptake in primary samples further validated the superiority of 11C-MET.
Conclusion
These data suggest that 11C-MET might be a versatile biomarker for myeloma superior to routine functional imaging with 18F-FDG regarding diagnosis, risk stratification, prognosis and discrimination of tumor subtypes.
Experimental evidence suggests that ubiquitin-protein ligases regulate a number of cellular processes involved in tumorigenesis. We analysed the role of the E3 ubiquitin-protein ligase HUWE1 for pathobiology of multiple myeloma (MM), a still incurable blood cancer. mRNA expression analysis indicates an increase in HUWE1 expression levels correlated with advanced stages of myeloma. Pharmacologic as well as RNAi-mediated HUWE1 inhibition caused anti-proliferative effects in MM cell lines in vitro and in an MM1.S xenotransplantation mouse model. Cell cycle analysis upon HUWE1 inhibition revealed decreased S phase cell fractions. Analyses of potential HUWE1-dependent molecular functions did not show involvement in MYC-dependent gene regulation. However, HUWE1 depleted MM cells displayed increased DNA tail length by comet assay, as well as changes in the levels of DNA damage response mediators such as pBRCA1, DNA-polymerase beta, gamma H2AX and Mcl-1. Our finding that HUWE1 might thus be involved in endogenous DNA repair is further supported by strongly enhanced apoptotic effects of the DNA-damaging agent melphalan in HUWE1 depleted cells in vitro and in vivo. These data suggest that HUWE1 might contribute to tumour growth by endogenous repair of DNA, and could therefore potentially be exploitable in future treatment developments.
The cancer stem cell hypothesis is a cancer development model which elicited great interest in the last decades stating that cancer heterogeneity arises from a stem cell through asymmetrical division. The Cancer Stem Cell subset is described as the only population to be tumorigenic and having the potential to renew. Conventional therapy often fails to eradicate CSC resulting in tumor relapse. Consequently, it is of great inter-est to eliminate this subset of cells to provide the best patient outcome. In the last years several approaches to target CSC were developed, one of them being immunotherapeu-tic targeting with antibodies. Since markers associated with CSC are also expressed on normal stem cells or healthy adjacent tissue in colorectal cancer, dual targeting strate-gies are preferred over targeting only a single antigen. Subsequently, the idea of dual targeting two CSC markers in parallel by a newly developed split T cell-engaging anti-body format termed as Hemibodies emerged. In a preliminary single cell RNA sequenc-ing analysis of colorectal cancer cells CD133, CD24, CD166 and CEA were identified as suitable targets for the combinatorial targeting strategy. Therefore, this study focused on trispecific and trivalent Hemibodies comprising a split binding moiety against CD3 and a binding moiety against either CD133, CD24, CD166 or CEA to overcome the occurrence of resistance and to efficiently eradicate all tumor cells including the CSC compartment. The study showed that the Hemibody combinations CD133xCD24, CD133xCD166 and CD133xCEA are able to eliminate double positive CHO cells with high efficacy while having a high specificity indicated by no killing of single antigen positive cells. A thera-peutic window ranging between one to two log levels could be achieved for all combina-tions mentioned above. The combinations CD133xCD24 and CD133xCD166 further-more proved its efficacy and specificity on established colorectal cancer cell lines. Be-sides the evaluation of specificity and efficacy the already introduced 1st generation of Hemibodies could be improved into a 2nd generation Hemibody format with increased half-life, stability and production yield. In future experiments the applicability of above-mentioned Hemibodies will be proven on patient-derived micro tumors to also include variables like tumor microenvironment and infiltration.
Background: Stimulation of CD40 can augment anti-cancer T cell immune responses by triggering effective activation and maturation of antigen-presenting cells (APCs). Although CD40 agonists have clinical activity in humans, the associated systemic activation of the immune system triggers dose-limiting side-effects.
Methods: To increase the tumor selectivity of CD40 agonist-based therapies, we developed an approach in which soluble trimeric CD40L (sCD40L) is genetically fused to tumor targeting antibody fragments, yielding scFv: CD40L fusion proteins. We hypothesized that scFv: CD40L fusion proteins would have reduced CD40 agonist activity similar to sCD40L but will be converted to a highly agonistic membrane CD40L-like form of CD40L upon anchoring to cell surface exposed antigen via the scFv domain.
Results: Targeted delivery of CD40L to the carcinoma marker EpCAM on carcinoma cells induced dose-dependent paracrine maturation of DCs similar to 20-fold more effective than a non-targeted control scFv: CD40L fusion protein. Similarly, targeted delivery of CD40L to the B cell leukemia marker CD20 induced effective paracrine maturation of DCs. Of note, the CD20-selective delivery of CD40L also triggered loss of cell viability in certain B cell leukemic cell lines as a result of CD20-induced apoptosis.
Conclusions: Targeted delivery of CD40L to cancer cells is a promising strategy that may help to trigger cancer-localized activation of CD40 and can be modified to exert additional anti-cancer activity via the targeting domain.
Der Erfolg der allogenen hämatopoetischen Stammzelltransplantation (HSZT) als Immuntherapie basiert neben den Minorantigendifferenzen zwischen Spender und Empfänger entscheidend auf einer spendervermittelten Immunität gegen tumorassoziierte Antigene (TAA), über deren Herkunft und Frequenz bei gesunden Blutspendern die derzeitige Studienlage kaum Informationen bietet. Da für viele klinisch relevante TAA eine Expression im fetalen und plazentaren Gewebe bekannt ist, wurde in dieser Arbeit die Schwangerschaft als möglicher Auslöser dieser T-lymphozytären Gedächtnisimmunantworten im Sinne eines Tumor- und Transplantationsmodells untersucht.
Hierfür wurden insgesamt 114 gesunde Blutspender in drei Subgruppen aus 38 Frauen mit negativer Schwangerschaftsanamnese, 38 Frauen mit positiver Geburtenanamnese und 38 Männern in einer Querschnittsstudie betrachtet, daneben wurden 44 Frauen longitudinal während und nach ihrer ersten Schwangerschaft untersucht. Dabei wurden die CD8-positiven T-Lymphozyten der Probanden isoliert, mit Peptiden der vier klinisch relevanten TAA HER2/neu (human epidermal growth factor receptor 2), MUC1 (Mucin 1), PRAME (preferentially expressed antigen of melanoma) und WT1 (Wilms tumor protein 1) stimuliert und die Produktion von IFN-γ-mRNA mittels RT-qPCR gemessen. Daneben wurden zum Vergleich durchflusszytometrische und ELISPOT-basierte Verfahren durchgeführt.
Bei den gesunden Blutspendern konnten CD8-positive Gedächtnisimmunantworten von niedriger und/oder hoher funktioneller Avidität gegen alle vier untersuchten TAA gemessen werden: Die Frequenz der dabei als „positiv“ definierten Immunantworten betrug bei HER2/neu 5 %, bei MUC1 14 %, bei PRAME 7 % und bei WT1 15 %. Männer wiesen insgesamt höhere absolute Level der Immunantworten gegen die untersuchten TAA auf, was auf eine testikuläre Expression dieser Antigene zurückzuführen sein könnte. In der Longitudinalanalyse bei den erstschwangeren Frauen ließen sich die stärksten Immunantworten zu Beginn der Schwangerschaft nachweisen, so dass es hier zu einem „Boost“ präexistenter TAA-spezifischer Autoimmunität zu kommen scheint. Durch das immunsuppressive hormonelle Milieu im Verlauf der Schwangerschaft und den Verlust der Zielantigene der feto-plazentaren Einheit durch die Geburt und Nachgeburt scheint diese Immunität aber nicht zu persistieren. Dadurch erklärt sich auch die Beobachtung, dass Frauen mit einer positiven Geburtenanamnese keine stärkeren Immunantworten gegen die untersuchten TAA aufwiesen als Frauen mit einer negativen Schwangerschaftsanamnese. Die Schwangerschaft hinterlässt diesbezüglich also ohne die Anwesenheit der vermittelnden Antigene keinen regelhaft bleibenden Effekt.
Diese Resultate decken sich mit Beobachtungen aus der Tumorimmuntherapie, bei denen Vakzinierungen gegen TAA zwar eine kurzfristige Immunität generieren konnten, die aber nicht persistierte. Im Rahmen der HSZT kann eine solche TAA-spezifische Immunität vom Spender auf den Empfänger transferiert werden und vermag dann aufgrund des proinflammatorischen Immunmilieus sehr wohl zu expandieren und in einem begrenzten Ausmaß auch zu persistieren.
Dementsprechend ergeben sich aus den in dieser Arbeit gewonnenen Resultaten relevante Implikationen für die allogene und in geringerem Ausmaß die autologe HSZT, daneben aber auch für innovative Tumortherapien wie die Immuncheckpoint-Blockade, da die Persistenz von tumorspezifischer Immunität letztendlich hochrelevant für eine langfristige Tumorkontrolle und damit für ein tumorfreies Überleben ist. Das vorliegende Modell trägt somit zum Verständnis der komplexen immunregulatorischen Vorgänge bei der Tumorkontrolle bei. Ob die hierbei aufgezeigten Immunantworten generell zu einer verbesserten TAA-spezifischen Immunrekonstitution und konsekutiv zu einem besseren klinischen Ergebnis beitragen, bleibt offen und wird in klinischen Studien geklärt werden müssen.
Hintergrund
Die Diagnose Krebs und ihre Behandlung kann eine große Belastung für die Betroffenen darstellen. Neben körperlichen Beschwerden kann auch die Psyche in Mitleidenschaft gezogen werden. Fehlt es an entsprechenden Bewältigungsstrategien, kann der selbstbestimmte Tod als einziger Ausweg erscheinen.
Ziel und Fragestellung
Die vorliegende Übersichtsarbeit zur Suizidalität bei Krebspatienten befasst sich mit einem Thema, das in der Forschung und Praxis in Deutschland nur wenig Aufmerksamkeit findet.
Material und Methoden
Eine themenbezogene Literaturrecherche stellt die Basis der Arbeit dar.
Ergebnisse
Todeswünsche unter Krebspatienten sind nicht selten und können Suizidgedanken/-absichten beinhalten. Psychische Beschwerden, insbesondere Hoffnungslosigkeit und Depression, sind ernstzunehmende Risikofaktoren. Das Erkennen einer hohen psychischen Belastung/von Todeswünschen ist ein wichtiger Aspekt für die Suizidprävention. Für die Praxis empfiehlt sich zunächst die Verwendung von Fragebögen. Bei auffälligen Werten muss die Suizidalität proaktiv in einem persönlichen Gespräch exploriert werden. Betroffene sind meist ambivalent bezüglich ihrer Entscheidung für oder gegen das Leben. Dies stellt eine große Chance für Interventionen dar.
Schlussfolgerungen
Suizidalität kann verhindert werden, wenn die hohe Belastung erkannt wird. Bereits das Gespräch zwischen Arzt und Patient über Todeswünsche kann eine erste Entlastung darstellen.
Background: The primary aim of this pilot study was to determine the feasibility and safety of an adoptive transfer and in vivo expansion of human haploidentical gamma delta T lymphocytes.
Methods: Patients with advanced haematological malignancies who are not eligible for allogeneic transplantation received peripheral blood mononuclear cells from half-matched family donors. For that, a single unstimulated leukapheresis product was incubated with both the anti-CD4 and anti-CD8 antibodies conjugated to paramagnetic particles. The depletion procedure was performed on a fully automated CliniMACS (R) device according to the manufacturer's instructions. On average, patients received 2.17 x 10(6)/kg (range 0.9-3.48) γδ T cells with <1% CD4-or CD8-positive cells remaining in the product. All patients received prior lymphopenia-inducing chemotherapy (fludarabine 20-25 mg/m(2) day -6 until day -2 and cyclophosphamide 30-60 mg/kg day -6 and -5) and were treated with 4 mg zoledronate on day 0 and 1.0x10(6) IU/m(2) IL-2 on day +1 until day +6 for the induction of gamma delta T cell proliferation in vivo.
Results: This resulted in a marked in vivo expansion of donor γδ T cells and, to a lower extent, natural killer cells and double-negative αβ T cells (mean 68-fold, eight-fold, and eight-fold, respectively). Proliferation peaked by around day +8 and donor cells persisted up to 28 days. Although refractory to all prior therapies, three out of four patients achieved a complete remission, which lasted for 8 months in a patient with plasma cell leukaemia. One patient died from an infection 6 weeks after treatment.
Conclusion: This pilot study shows that adoptive transfer and in vivo expansion of haploidentical γδ T lymphocytes is feasible and suggests a potential role of these cells in the treatment of haematological diseases.
Background: Renal cell carcinoma (RCC) is divided into three major histopathologic groups—clear cell (ccRCC), papillary (pRCC) and chromophobe RCC (chRCC). We performed a comprehensive re-analysis of publicly available RCC datasets from the TCGA (The Cancer Genome Atlas) database, thereby combining samples from all three subgroups, for an exploratory transcriptome profiling of RCC subgroups.
Materials and Methods: We used FPKM (fragments per kilobase per million) files derived from the ccRCC, pRCC and chRCC cohorts of the TCGA database, representing transcriptomic data of 891 patients. Using principal component analysis, we visualized datasets as t-SNE plot for cluster detection. Clusters were characterized by machine learning, resulting gene signatures were validated by correlation analyses in the TCGA dataset and three external datasets (ICGC RECA-EU, CPTAC-3-Kidney, and GSE157256).
Results: Many RCC samples co-clustered according to histopathology. However, a substantial number of samples clustered independently from histopathologic origin (mixed subgroup)—demonstrating divergence between histopathology and transcriptomic data. Further analyses of mixed subgroup via machine learning revealed a predominant mitochondrial gene signature—a trait previously known for chRCC—across all histopathologic subgroups. Additionally, ccRCC samples from mixed subgroup presented an inverse correlation of mitochondrial and angiogenesis-related genes in the TCGA and in three external validation cohorts. Moreover, mixed subgroup affiliation was associated with a highly significant shorter overall survival for patients with ccRCC—and a highly significant longer overall survival for chRCC patients.
Conclusions: Pan-RCC clustering according to RNA-sequencing data revealed a distinct histology-independent subgroup characterized by strengthened mitochondrial and weakened angiogenesis-related gene signatures. Moreover, affiliation to mixed subgroup went along with a significantly shorter overall survival for ccRCC and a longer overall survival for chRCC patients. Further research could offer a therapy stratification by specifically addressing the mitochondrial metabolism of such tumors and its microenvironment.
Expansion microscopy (ExM) enables super-resolution fluorescence imaging on standard microscopes by physical expansion of the sample. However, the investigation of interactions between different organisms such as mammalian and fungal cells by ExM remains challenging because different cell types require different expansion protocols to ensure identical, ideally isotropic expansion of both partners. Here, we introduce an ExM method that enables super-resolved visualization of the interaction between NK cells and Aspergillus fumigatus hyphae. 4-fold expansion in combination with confocal fluorescence imaging allows us to resolve details of cytoskeleton rearrangement as well as NK cells' lytic granules triggered by contact with an RFP-expressing A. fumigatus strain. In particular, subdiffraction-resolution images show polarized degranulation upon contact formation and the presence of LAMP1 surrounding perforin at the NK cell-surface post degranulation. Our data demonstrate that optimized ExM protocols enable the investigation of immunological synapse formation between two different species with so far unmatched spatial resolution.
Oesophageal involvement in mucous membrane pemphigoid is considered rare, but it may be underdiagnosed. To assess the incidence of oesophageal involvement in a group of patients with newly diagnosed mucous membrane pemphigoid we retrospectively analysed the medical records of 30 consecutive patients with mucous membrane pemphigoid diagnosed between 2006 and 2016 at the Department of Dermatology, University Hospital Würzburg. Twenty-one patients (70%) reported symptoms indicative of oesophageal mucous membrane pemphigoid. Twelve patients (40%) underwent oesophagogastroduodenoscopy, and oesophageal pathology compatible with mucous membrane pemphigoid was endoscopically found in 9 cases (30%). In all patients indirect and direct immunofluorescence were performed. Patients with and without oesophageal involvement did not differ with regard to the results of indirect immunofluorescence on salt-split human skin and monkey oesophagus. Study results demonstrate the necessity of a standardized diagnostic work-up, including adequate tissue samples for direct immunofluorescence, to prevent underdiagnosis of oesophageal mucous membrane pemphigoid.
Allogeneic hematopoietic stem cell transplantation (HSCT) is often the only effective treatment for patients with hematological malignancies, but its curative potential is often limited by the development of acute or chronic graft-versus-host disease (GvHD). Although extensive immunosuppressive therapy is highly efficient in the prevention or treatment of GvHD, it greatly increases the risk for life-threatening opportunistic fungal or viral infections and the recurrence of malignant disease. The possibility to selectively deplete alloreactive T cells from donor grafts prior or after transplantation would greatly diminish the need for immunosuppressive therapy in the transplant recipient and thereby greatly improve its clinical outcome. The molecular chaperone heat shock protein of 90 kDa (Hsp90) has been previously shown to stabilize many signal transduction proteins involved in T lymphocyte activation and proliferation and is furthermore able to exert anti-apoptotic effects in different cell types. The aim of this study was therefore to investigate the possibility to selectively target activated, proliferating T cells in lymphocyte populations by inhibition of Hsp90, without compromising viability and function of non-reactive T cell populations including pathogen-specific T lymphocytes. It could be shown in this work, that activated T cells are indeed more prone to apoptotic cell death in the presence of Hsp90 inhibitors than resting cells and that treatment of mixed lymphocyte cultures with such inhibitors eliminates the proliferation of alloreactive cells. In contrast, T cells remaining in a resting state during inhibitor treatment remain viable and also display functional virus-specific responses after inhibitor removal. These data suggest, that Hsp90 could represent a novel target for selective depletion of alloreactive T cells and that application of Hsp90 inhibitors could be a potential approach to prevent or treat GvHD without impairing pathogen-specific T cell immunity. In the second part of this work, the immune responses to strictly defined antigens of the opportunistic pathogenic fungus Aspergillus fumigatus were characterized. Opportunistic fungal infections are highly prevalent in immunocompromized and immunosuppressed individuals, especially in HSCT recipients suffering from GvDH. Although antifungal treatment is permanently improved, invasive fungal infections are still often fatal. In healthy individuals clinical disease is rare, because innate and adaptive immunity act in conjunction to protect the host. Therefore one possible strategy to prevent and treat life-threatening fungal infections in immunocompromized patients is to improve host resistance by augmenting the antifungal functions of the immune system, for example by vaccination or adoptive transfer of antigen-specific T cells. Based on previous findings, the objective of this dissertation was to identify and characterize distinct immunogenic A. fumigatus antigens that could be used for clinical application like vaccination or ex vivo generation of antigen-specific T cells and to characterize the interaction of this antigen-specific lymphocytes with cells of the innate immune system. First, memory T cell responses to different recombinant A. fumigatus proteins in healthy individuals were evaluated. The majority of tested donors displayed stable CD4+ TH1 responses to the Crf1 protein, whereas responses to the other antigens tested could only be detected in a limited number of donors, qualifying Crf1 as potential candidate antigen for clinical use. It was also possible to identify an immunodominant MHC class II DRB1*04-restricted epitope of Crf1 and to generate T cell clones specific for this epitope. This Crf1-specific T cell clones could be specifically activated by dendritic cells fed with synthetic peptide, recombinant protein or germinating A. fumigatus conidia or outgrown hyphae. Interestingly, these A. fumigatus-specific T cell clones also responded to stimulation with Candida albicans, which likewise causes opportunistic infections in immunocompromized patients and encodes for a glucosyltransferase similar to A. fumigatus Crf1. It was also possible to show that supernatant harvested from activated Crf1-specific T cell cultures was able to significantly increase fungal killing by monocytes. These data indicate that the specified FHT epitope of the A. fumigatus protein Crf1 could be potentially used as antigen for vaccination protocols or for the generation of Aspergillus-specific effector T cells for adoptive transfer.
Recent advances in the field of cancer immunotherapy have enabled this therapeutic approach to enter the mainstream of modern cancer treatment. In particular, adoptive T cell therapy (ACT) is a potentially powerful immunotherapy approach that relies on the administration of tumor-specific T cells into the patient. There are several strategies to obtain tumor-reactive cytotoxic T lymphocytes (CTLs), which have already been shown to induce remarkable responses in the clinical setting. However, there are concerns and limitations regarding the conventional approaches to obtain tumor-reactive T cells, such as accuracy of the procedure and reproducibility. Therefore, we aimed to develop two approaches to improve the precision and efficacy of tumor-reactive T cells therapy. These two techniques could constitute effective, safe and broadly applicable alternatives to the conventional methods for obtaining tumor-specific CTLs.
The first approach of this study is the so called “Doublet Technology”. Here, we demonstrate that peptide-human leukocyte antigen-T cell receptor (pHLA-TCR) interactions that involve immune reactive peptides are stable and strong. Therefore, the CTLs that are bound by their TCR to tumor cells can be selected and isolated through FACS-based cell sorting taking advantage of this stable interaction between the CTLs and the target cells. The CTLs from acute myeloid leukemia (AML) patients obtained with this technique show cytolytic activity against blast cells suggesting a potential clinical use of these CTLs. “Doublet Technology” offers a personalized therapy in which there is no need for a priori knowledge of the exact tumor antigen.
The second approach of this study is the Chimeric Antigen Receptor (CAR) Technology. We design several CARs targeting the B-Cell Maturation Antigen (BCMA). BCMA CAR T cells show antigen-specific cytolytic activity, production of cytokines including IFN-γ and IL-2, as well as productive proliferation. Although we confirm the presence of soluble BCMA in serum of multiple myeloma (MM) patients, we demonstrate that the presence of soluble protein does not abrogate the efficacy of BCMA CAR T cells suggesting that BCMA CAR T cells can be used in the clinical setting to treat MM patients. The high antigen specificity of CAR T cells allows efficient tumor cell eradication and makes CAR Technology attractive for broadly applicable therapies.
Humane gamma delta T-Zellen stellen eine Gruppe von T-Lymphozyten dar, die neben NKT- und NK-Zellen auch als lymphatische Effektorzellen der angeborenen Immunität beschrieben werden. Nach ihrer Stimulierung tragen sie, in Analogie zu den NK-Zellen, zur schnellen, unspezifischen Infektionsabwehr bei. In dieser Arbeit wurde erstmals der Einfluss von ds-RNS (Poly (I:C)) auf humane gamma delta T-Zellen untersucht. Diese Substanz simuliert in vivo und in vitro die Infektion mit einem Virus und übt pleiotrope Effekte auf eine Reihe von verschiedenen Zellen aus, u. a. auch auf NK-Zellen. Diese Arbeit beschreibt die potente kostimulierende Wirkung von Poly (I:C) auch auf gamma delta T-Zellen: Es induziert deren partielle Aktivierung, sichtbar am Oberflächenmolekül CD69. Daneben stimuliert die ds-RNS die Antigen-spezifische gamma delta T-Zell-Antwort, nachweisbar an einer Zunahme der Proliferation. Die Sekretion von IFN-gamma durch gamma delta T-Zellen ist jedoch nicht gesteigert. Die Wirkungsmechanismen von Poly (I:C) auf gamma delta T-Zellen sind, im Gegensatz zu den NK-Zellen, in erster Linie indirekt. Durch Depletion von CD11c-positiven dendritischen Zellen wird die Poly (I:C)-vermittelte Aktivierung der gamma delta T-Zellen vollständig aufgehoben. Ebenso kann durch Neutralisierung von Typ I Interferonen, die v. a. nach Poly (I:C)-vermittelter Stimulation dieser DCs sezerniert werden, größtenteils eine Aufhebung der Poly (I:C)-vermittelten CD69-Expression auf gamma delta T-Zellen erzielt werden. Diese Ergebnisse legen eine Antigen-unabhängige, schnelle Aktivierung von gamma delta T-Zellen im Rahmen viraler Infektionen nahe.
Das Multiple Myelom ist eine komplexe Erkrankung, dessen Tumorbiologie noch immer nicht in Gänze verstanden ist. Mit dem Heavy Light Chain Assay (Hevylite®) war es erstmals möglich, mit spezifischen Antikörpern nicht nur zwischen den Klassen intakter Immunglobuline, sondern auch zwischen kappa- und lambda-Isotyp zu differenzieren.
Dies ist in der Behandlung von Patient*innen mit Multiplem Myelom sehr nützlich, um das vom Tumor produzierte klonale Immunglobulin von den funktionalen Immunglobulinen getrennt zu quantifizieren. Dadurch sollen die Tumorlast und die einhergehende Immunsuppression genauer erfasst werden.
Den zusätzlichen Nutzen für Diagnostik und Therapiemonitoring des Multiplen Myeloms untersuchen wir in dieser Arbeit anhand von Daten einer multizentrischen, randomisierten Phase 3- Medikamentenstudie (DSMM XIV) mit dem Vorteil, hierdurch eine große und weitgehend einheitlich behandelte Kohorte und Zugang zu modernen Messmethoden zu haben.
Wir bestätigen, dass das Heavy Light Chain Assays insbesondere zur Erkennung von IgA-Myelomen eine hohe Sensitivität bei negativer Serumproteinelektrophorese hat. Weiterhin zeigen wir, dass je nach Zeitpunkt in der Therapie das Heavy Light Chain Assay ein höheres Risiko für einen Progress vorhersagt als bisher verwendete Methoden. Signifikante Unterschiede im progressionsfreien Überleben finden wir nicht nur je nach Höhe der kappa/lambda Heavy Light Chain-Ratio des involvierten Immunglobulins, sondern auch bei Suppression der nicht involvierten Heavy Light Chain. Zudem beschreiben wir eine hohe Korrelation zwischen hoch abnormaler kappa/lambda Heavy Light Chain-Ratio des involvierten Immunglobulins und positivem Minimal Residual Disease Status in der Durchflusszytometrie.
Wir empfehlen daher anhand unserer Ergebnisse, dass das Heavy Light Chain Assay einen Platz in der diagnostischen Routine erhält und als prognostischer Faktor zusätzlich in die Response-Kriterien integriert wird.
Der vorliegenden retrospektiven Untersuchung zum Stellenwert der strukturierten am-bulanten Nachsorge in der Rezidiverkennung von aggressiven B-NHL liegen Patientendaten der Medizinischen Klinik und Poliklinik II der Universität Würzburg aus den Jahren 1991- 2003 zugrunde. Bei den eingeschlossenen 141 Patienten wurde im Zeitraum vom 01.01.1991 bis zum 31.12.2001 die Primärdiagnose eines aggressiven B-NHL ge-stellt und nach Erstlinientherapie mit Erreichen einer kompletten Remission die ambu-lante Nachsorge begonnen. Von den 35 ambulant diagnostizierten Rezidiven traten 46% innerhalb des ersten Jahres nach Beendigung der Therapie auf. Innerhalb der ersten fünf Jahre nach Therapieende waren 92% der ambulant diagnostizierten Wiedererkrankungen aufgetreten. Auffallend ist, dass nur 51% der Rezidive anlässlich eines vereinbarten Termins im Rahmen des Nachsorgeschemas entdeckt wurden. Die übrigen Patienten erschienen aufgrund von Symptomen außerhalb des empfohlenen Schemas. Das Wahrnehmen von Symptomen durch den Patienten zählt somit zu den Bestandteilen einer erfolgreichen Nachsorge. In der vorliegenden Untersuchung wurde dies signifikant bestätigt. 72% der Rezidivpatien-ten berichteten über mindestens eines der Symptome: B-Symptomatik, Lymphknoten-schwellung, Infekten, Schmerzen an Lymphknotenstationen oder Juckreiz. Bei den ge-sunden Patienten fanden sich diese Symptome nur in 20% der Fälle. Die Patientenschu-lung sollte daher neben der Anamnese einen Schwerpunkt der Nachsorge darstellen. Bei Auftreten der oben genannten Symptome sollen sich die Patienten außerhalb der emp-fohlenen Nachsorgeintervalle vorstellen. Ebenso nahm die körperliche Untersuchung einen hohen Stellenwert in der Rezidiver-kennung ein. 20 der 35 Rezidivpatienten hatten einen auffälligen Untersuchungsbefund, davon in 15 Fällen (43%) vergrößert tastbare Lymphknoten. Eine Erhöhung der Laktatdehydrogenase (LDH) trat bei 10 der 35 Rezidivpatienten (29%) auf. Durch die Kombination aus Anamnese, körperlicher Untersuchung und LDH-Bestimmung wurden 34 von 35 Rezidivpatienten erkannt. Empfohlen werden kann aufgrund der analysierten Daten eine Nachsorgestruktur mit engmaschigen Kontrollen im Abstand von drei Monaten innerhalb des ersten Jahres nach Therapieende und darauf folgende halbjährliche Kontrollen für weitere vier Jahre. Die Sonographie, ein Routineverfahren der Nachsorge, zeigte sich in der vorliegenden Untersuchung als geeignetes Verfahren zur Bestätigung eines Rezidivs. Bei 28 von 30 untersuchten Rezidivpatienten wurde ein pathologischer Befund im Sinne eines Rezi-divs erhoben. Von der routinemäßigen Bildgebung mithilfe der Schnittbildverfahren CT oder MRT profitierte nur ein Patient, welcher ausschließlich durch die Bildgebung auf-fiel. Diese Verfahren sollten nicht routinemäßig, sondern erst bei Vorliegen auffälliger Befunde durchgeführt werden. Der Nutzen der strukturierten ambulanten Nachsorge scheint aufgrund der vorliegenden Arbeit nur begrenzt geeignet Rezidive zu erkennen, da 49% der Rezidive außerhalb der strukturierten Nachsorge diagnostiziert wurden. Sie bietet aber zugleich ein notwendi-ges Raster für die systematische Nachbetreuung der Patienten und sollte durch eine gründliche Patientenschulung ergänzt werden, um die Patienten frühzeitig auf suspekte Symptome aufmerksam zu machen. Ein weiteres Argument für die strukturierte Nach-sorge stellt die psychische Belastungssituation dar, in der sich die meisten Patienten nach der Therapie eines Lymphoms befinden. Die Patienten bekommen nicht nur früh-zeitig Gewissheit, wenn sie erneut erkranken, sondern erfahren auch Beruhigung und Bestätigung, wenn die Erkrankung nicht wieder aufgetreten ist.
Reliable standards and criteria for somatostatin receptor (SSTR) positron emission tomography (PET) are still lacking. We herein propose a structured reporting system on a 5-point scale for SSTR-PET imaging, titled SSTR-RADS version 1.0, which might serve as a standardized assessment for both diagnosis and treatment planning in neuroendocrine tumors (NET). SSTR-RADS could guide the imaging specialist in interpreting SSTR-PET scans, facilitate communication with the referring clinician so that appropriate work-up for equivocal findings is pursued, and serve as a reliable tool for patient selection for planned Peptide Receptor Radionuclide Therapy.
(1) Background: The aim of our study was to identify specific risk factors for fatal outcome in critically ill COVID-19 patients. (2) Methods: Our data set consisted of 840 patients enclosed in the LEOSS registry. Using lasso regression for variable selection, a multifactorial logistic regression model was fitted to the response variable survival. Specific risk factors and their odds ratios were derived. A nomogram was developed as a graphical representation of the model. (3) Results: 14 variables were identified as independent factors contributing to the risk of death for critically ill COVID-19 patients: age (OR 1.08, CI 1.06–1.10), cardiovascular disease (OR 1.64, CI 1.06–2.55), pulmonary disease (OR 1.87, CI 1.16–3.03), baseline Statin treatment (0.54, CI 0.33–0.87), oxygen saturation (unit = 1%, OR 0.94, CI 0.92–0.96), leukocytes (unit 1000/μL, OR 1.04, CI 1.01–1.07), lymphocytes (unit 100/μL, OR 0.96, CI 0.94–0.99), platelets (unit 100,000/μL, OR 0.70, CI 0.62–0.80), procalcitonin (unit ng/mL, OR 1.11, CI 1.05–1.18), kidney failure (OR 1.68, CI 1.05–2.70), congestive heart failure (OR 2.62, CI 1.11–6.21), severe liver failure (OR 4.93, CI 1.94–12.52), and a quick SOFA score of 3 (OR 1.78, CI 1.14–2.78). The nomogram graphically displays the importance of these 14 factors for mortality. (4) Conclusions: There are risk factors that are specific to the subpopulation of critically ill COVID-19 patients.
The optimal treatment sequence of tyrosine kinase inhibitor (TKI)-based therapy in patients with hepatocellular carcinoma (HCC) remains unclear. Therefore, sequential systemic therapy after first-line therapy with sorafenib or lenvatinib was compared in a retrospective real-world cohort. In total, 164 patients with HCC were included. Child B cirrhosis was present in 26 patients (16.5%), whereas 132 patients (83.5%) had preserved liver function. In total, 72 patients (44%) discontinued systemic therapy after first-line therapy while 51 (31%) and 31 (19%) patients received 2 or more treatment lines. Most notably, median overall survival (mOS) was influenced by liver functional status and patient performance status at the beginning of first-line therapy. Patients receiving a sequential therapy regimen had significantly longer mOS compared to patients that discontinued systemic therapy after omitting first-line treatment. The choice of the initial TKI did not impact mOS. A clear deterioration of liver function could be observed during the course of TKI-based treatment.
