Peroral endoscopic myotomy (POEM) is a new endoscopic treatment for achalasia with very good short-term results in adults. Data about POEM in pediatric patients are missing. We present the case of a 10-year-old male patient with type I (classic) achalasia, successfully treated with POEM. The procedure was accomplished in a similar fashion to the technique used in adults. Short-term results were fine, with a complete control of dysphagia and absence of reflux. We suggest that POEM is a suitable option in pediatric patients—similar to adults—but long-term results must be awaited.
Objective: To investigate the impact of the phophodiesterase-4 inhibition (PD-4-I) with rolipram on hepatic integrity in lipopolysaccharide (LPS) induced hyperinflammation. Materials and Methods: Liver microcirculation in rats was obtained using intravital microscopy. Macrohemodynamic parameters, blood assays, and organs were harvested to determine organ function and injury. Hyperinflammation was induced by LPS and PD-4-I rolipram was administered intravenously one hour after LPS application. Cell viability of HepG2 cells was measured by EZ4U-kit based on the dye XTT. Experiments were carried out assessing the influence of different concentrations of tumor necrosis factor alpha (TNF-α) and LPS with or without PD-4-I. Results: Untreated LPS-induced rats showed significantly decreased liver microcirculation and increased hepatic cell death, whereas LPS + PD-4-I treatment could improve hepatic volumetric flow and cell death to control level whithout influencing the inflammatory impact. In HepG2 cells TNF-α and LPS significantly reduced cell viability. Coincubation with PD-4-I increased HepG2 viability to control levels. The heme oxygenase 1 (HO-1) pathway did not induce the protective effect of PD-4-I. Conclusion: Intravenous PD-4-I treatment was effective in improving hepatic microcirculation and hepatic integrity, while it had a direct protective effect on HepG2 viability during inflammation.
Bariatric operations in obese patients with type 2 diabetes often improve diabetes before weight loss is observed. In patients mainly Roux-en-Y-gastric bypass with partial stomach resection is performed. Duodenojejunal bypass (DJB) and ileal interposition (IIP) are employed in animal experiments. Due to increased glucose exposition of L-cells located in distal ileum, all bariatric surgery procedures lead to higher secretion of antidiabetic glucagon like peptide-1 (GLP-1) after glucose gavage. After DJB also downregulation of Na\(^{+}\)-D-glucose cotransporter SGLT1 was observed. This suggested a direct contribution of decreased glucose absorption to the antidiabetic effect of bariatric surgery. To investigate whether glucose absorption is also decreased after IIP, we induced diabetes with decreased glucose tolerance and insulin sensitivity in male rats and investigated effects of IIP on diabetes and SGLT1. After IIP, we observed weight-independent improvement of glucose tolerance, increased insulin sensitivity, and increased plasma GLP-1 after glucose gavage. The interposed ileum was increased in diameter and showed increased length of villi, hyperplasia of the epithelial layer, and increased number of L-cells. The amount of SGLT1-mediated glucose uptake in interposed ileum was increased 2-fold reaching the same level as in jejunum. Thus, improvement of glycemic control by bariatric surgery does not require decreased glucose absorption.
In vitro evidence for senescent multinucleated melanocytes as a source for tumor-initiating cells

Oncogenic signaling in melanocytes results in oncogene-induced senescence (OIS), a stable cell-cycle arrest frequently characterized by a bi-or multinuclear phenotype that is considered as a barrier to cancer progression. However, the long-sustained conviction that senescence is a truly irreversible process has recently been challenged. Still, it is not known whether cells driven into OIS can progress to cancer and thereby pose a potential threat. Here, we show that prolonged expression of the melanoma oncogene N-RAS\(^{61K}\) in pigment cells overcomes OIS by triggering the emergence of tumor-initiating mononucleated stem-like cells from senescent cells. This progeny is dedifferentiated, highly proliferative, anoikis-resistant and induces fast growing, metastatic tumors. Our data describe that differentiated cells, which are driven into senescence by an oncogene, use this senescence state as trigger for tumor transformation, giving rise to highly aggressive tumor-initiating cells. These observations provide the first experimental in vitro evidence for the evasion of OIS on the cellular level and ensuing transformation.
Cell-based strategies represent a new frontier in the treatment of immune-mediated disorders. However, the paucity of markers for isolation of molecularly defined immunomodulatory cell populations poses a barrier to this field. Here, we show that ATP-binding cassette member B5 (ABCB5) identifies dermal immunoregulatory cells (DIRCs) capable of exerting therapeutic immunoregulatory functions through engagement of programmed cell death 1 (PD-1). Purified Abcb5\(^+\) DIRCs suppressed T cell proliferation, evaded immune rejection, homed to recipient immune tissues, and induced Tregs in vivo. In fully major-histocompatibility-complex-mismatched cardiac allotransplantation models, allogeneic DIRCs significantly prolonged allograft survival. Blockade of DIRC-expressed PD-1 reversed the inhibitory effects of DIRCs on T cell activation, inhibited DIRC-dependent Treg induction, and attenuated DIRC-induced prolongation of cardiac allograft survival, indicating that DIRC immunoregulatory function is mediated, at least in part, through PD-1. Our results identify ABCB5\(^+\) DIRCs as a distinct immunoregulatory cell population and suggest promising roles of this expandable cell subset in cellular immunotherapy.
Eine Nabelhernie stellt im Kindesalter eine angeborene Faszienlücke im Bereich der ventralen Bauchwand dar. Material und Methoden: Über einen Zeitraum von über zehn Jahren wurden in der kinderchirurgischen Abteilung der Klinik und Poliklinik für Allgemein-, Viszeral-, Gefäß- und Kinderchirurgie der Universität Würzburg 518 Patienten mit Nabelhernien operiert, wobei 499 Patienten nach Ausschlusskriterien in der Studie evaluiert wurden. Ergebnisse: Durch die Direktnaht und Sicherung mittels einer zweiten Fasziennaht nach semizirkulärer, subumbilicaler Inzision nach Spitzy konnte eine niedrige Rezidivquote von 1% (n=5 von n=499) erzielt werden. Die jungen Patienten waren im Durchschnitt 33,8 Monate alt und zu 44,1% weiblich und zu 55,9% männlich. Im Vergleich hierzu lag die Rezidivquote in der Erwachsenenchirurgie nach Direktnaht bei 12,9% (n=4 von n=31 Direktnähten bei n=62 Nabelhernien). Das Durchschnittsalter der erwachsenen Patienten betrug 54,5 Jahre; 32,3% waren weiblich, 67,7% männlich. Schlussfolgerung: Aufgrund der Studienergebnisse stellt die Nabelhernienversorgung im Kindesalter durch Direktnaht und Zugang nach Spitzy das Mittel der Wahl und den Goldstandard dar, sobald dringliche Operationsindikationen wie eine Größenprogredienz, Schmerzsymptomatik oder Inkarzerationszeichen vorliegen.
Immunsuppression und Glucocorticoid-Einnahme wurden bereits als Risikofaktoren für die Entwicklung eines komplizierten Verlaufs der akuten Sigmadivertikulitis erkannt. Der zugrundeliegende molekulare Mechanismus ist jedoch noch nicht vollständig verstanden. Ziel dieser Arbeit war es den Glucocorticoid-induzierten TNF Rezeptor (GITR) und die Matrixmetalloproteinase 9 (MMP-9) als potentielles molekulares Bindeglied zwischen Immunsuppression und der Entwicklung einer komplizierten Form der Divertikulitis zu untersuchen.
Hiefür erfolgte die Analyse von GITR und MMP-9 auf Proteinebene (Immunhistochemie, Immunfluoreszenz, Doppelmarkierungen) und mRNA-Ebene (semiquantitative RT-PCR) im Colon sigmoideum von Patienten, welches aufgrund einer Sigmadivertikulitis (kompliziert und unkompliziert) reseziert worden war. Abschließend folgte die Korrelation mit den klinischen Parametern. Die Wertigkeit der GITR-Expression als diagnostischer Test wurde mittels ROC-Analyse untersucht.
Die durchgeführten Untersuchungen zeigten eine hohe GITR- und MMP-9- Expression im entzündlichen Infiltrat bei komplizierten Sigmadivertikulitiden, während sich bei den unkomplizierten Formen nur eine geringe Expression nachweisen ließ. Die immunhistochemischen Ergebnisse sind auf mRNA-Ebene bestätigt worden. Die Immunfluoreszenz-Doppelfärbung zeigte eine deutliche Korrelation zwischen MMP- 9- und GITR-Expression (p<0,001). Eine hohe GITR-Expression war signifikant mit der Einnahme von Glucocorticoiden assoziiert (p<0,001). Die ROC- Analyse bestätigte die Überlegenheit von GITR als diagnostischer Test gegenüber Labortests (CRP, WBC) und klinischen Parametern (Körpertemperatur) bei der Differenzierung zwischen komplizierter und unkomplizierter Divertikulitis.
Die erhöhte MMP-9-Expression, welche durch GITR induziert wird, könnte die bereits bekannten Veränderungen der Kolonwand bei Patienten mit komplizierter Divertikulitis erklären. Darüber hinaus lassen diese Ergebnisse GITR als mögliches molekulares Bindeglied zwischen Glucocorticoid-Einnahme und komplizierten Verlaufsformen der Sigmadivertikulitis vermuten und unterstützen somit die frühzeitige Indikation zur elektiven Sigmaresektion bei Immunsuppression.
In der vorliegenden Arbeit wurde die Expression zweier intestinaler Stammzellmarker, LgR5 und DCAMKL-1, im ösophagealen Adenkarzinom mit und ohne Barrett-Ösophagus in Bezug auf die Stammzellhypothese untersucht.
Die Untersuchungen fanden an chirurgischen Resektaten und an ösophagealen Adenokarzinomzellen der Zelllinie OE-33 statt. Die Gewebeproben waren bei Operationen zur primären Entfernung von Ösophaguskarzinomen gewonnen worden und stammten von Patienten, die keine neoadjuvante antineoplastische Therapie erhalten hatten. Wurde auf den Gewebeschnitten neben dem Karzinomgewebe Barrett-Mukosa identifiziert, wurde das Karzinom als „Adenokarzinom mit Barrett-Epithel“ („EAC mit BE“; n = 41) klassifiziert, anderenfalls als „Adenokarzinom ohne Barrett-Epithel“ („EAC ohne BE“; n = 19). Die Identifikation der Barrett-Mukosa fand mikroskopisch und immunhistologisch (Marker: Cdx2) statt.
Zur Expressionsanalyse führten wir immunhistochemische Färbungen mit Antikörpern gegen LgR5 und DCAMKL-1 durch. Analysen auf Genebene fanden mittels RT-PCR statt. Es wurden Immunfluoreszenz-Doppelfärbungen mit LgR5 und dem Proliferationsmarker Ki-67 angefertigt. Die Ergebnisse wurden mit dem Tumorstadium und den 5-Jahres-Überlebensraten korreliert.
Eine LgR5-Expression wurde in 35 der 41 (85 %) Adenokarzinome mit Barrett- Epithel und in 16 der 19 (84%) Adenokarzinome ohne Barrett-Epithel gefunden. Der Anteil der LgR5-positiven Zellen lag bei den Adenokarzinomen mit Barrett Epithel bei 15 %, im angrenzenden Barrett-Epithel bei 32 % und bei den Adenokarzinomen ohne Barrett-Epithel bei 13 %. Karzinome mit und ohne Barrett-Epithel wiesen damit eine nicht signifikant unterschiedliche LgR5- Expression auf. Die Expression im Barrett-Epithel war im Vergleich dazu erhöht. Die Ergebnisse der RT-PCR auf mRNA-Ebene bestätigten die im Vergleich zum Adenokarzinomgewebe höhere LgR5-Expression im Barrett-Epithel (p = 0,0159). Für DCAMKL-1 zeigten sich durchweg negative Färbeergebnisse.
Mittels Immunfluoreszenz-Doppelfärbungen mit Antikörpern gegen LgR5 und Ki-67 ließen sich drei Zellpopulationen identifizieren: LgR5-positive, nicht proliferierende Zellen (LgR5+ / Ki-67−), LgR5-negative, proliferierende Zellen (LgR5– / Ki-67+) und wenige proliferierende, LgR5-positive Zellen (LgR5+ / Ki-67+). Bei der kleinen Subpopulation LgR5-positiver, proliferierender Zellen könnte es sich um Krebsstammzellen handeln. Die proliferierenden, LgR5- negativen Zellen können eher mit dem Modell der klonalen Selektion erklärt werden. Unsere Ergebnisse scheinen daher gut zu dem aktuellen Verständnis der Pathogenese des ösophagealen Adenokarzinoms zu passen, wobei das Modell der klonalen Selektion mit der Hypothese der Krebsstammzellen kombiniert wird. Eine höhere LgR5-Expression war in der univariaten Analyse mit einem schlechteren Überleben assoziiert. Patienten mit einem hohen Prozentsatz an LgR5-positiven Zellen wiesen eine schlechtere Prognose auf verglichen mit Patienten mit einem niedrigeren Prozentsatz LgR5-positiver Zellen. Dieser Zusammenhang gilt für Zellen sowohl im Karzinomgewebe als auch im Barrett-Epithel.
Wir haben gezeigt, dass der intestinale Stammzellmarker LgR5 – im Gegensatz zu DCAMKL-1 – im ösophagealen Adenokarzinom exprimiert wird. Die Expression scheint unabhängig vom Vorhandensein einer Barrett-Mukosa zu sein. Möglicherweise ist eine höhere Expression mit einer schlechteren Prognose assoziiert. Unsere Ergebnisse sind mit einem Modell für die Pathogenese des ösophagealen Adenokarzinoms vereinbar, das auf Krebsstammzellen basiert. LgR5 könnte dabei helfen, diese zu identifizieren. Das Aufzeigen neuer Ansatzpunkte für zielgerichtete Therapien könnte dabei helfen, neue wirksame Methoden zur Behandlung des ösophagealen Adenokarzioms zu entwickeln.
Hintergrund: In der präventiven und therapeutischen Behandlung von hypertrophen Narben und Keloiden hat sich die Behandlungsstrategie der Kompressionstherapie etabliert. Bisher konnte ein Druckoptimum der Kompressionsbekleidung von 25mmHg anhand der Kapillarkompressionstheorie ermittelt werden, welches an Kindern durchschnittlich erreicht wird. Langzeitstudien zeigten abweichendes Outcome der Therapie bei Kindern und Erwachsenen. In dieser Studie sollte nun herausgefunden werden, ob das unterschiedliche Ansprechen der Therapie in andersartigem Druckverhalten begründet ist.
Material und Methoden: Eingebracht werden konnten Messungen an 100 Kompressionstherapien, zugeordnet zu den Gruppen „Kinder“ und „Erwachsene“ mittels Kikuhime®-Drucksensor.
Analysiert und gegenübergestellt wurden die Werte anhand der Lokalisation der Messung, der Gewebeunterlage und der Tragedauer der Kompressionstherapien.
Ergebnisse und Diskussion: (1) In der Gesamtschau aller Messergebnisse besteht im Vergleich beider Gruppen kein signifikanter Unterschied hinsichtlich des Kompressionsdruckes.
(2) Nach spezieller Gegenüberstellung der Körperregionen Arm, Bein, Fuß und Stamm ergibt sich kein signifikanter Unterschied der Druckwerte.
(3) Hinsichtlich des Untergrundgewebes kann für die Gegenüberstellung beider Gruppen kein signifikanter Unterschied ermittelt werden.
(4) Ein signifikanter Unterschied wurde bei Betrachtung der Messergebnisse bezüglich der Tragedauer sowohl in der Gruppe der Erwachsenen als auch in der Gruppe der Kinder bestätigt. So wurden deutlich niedrigere Werte an alten Kompressionsanzügen ermittelt.
Zusammenfassung: Sowohl in der Gruppe der Kinder als auch in der Gruppe der Erwachsenen kann das Druckoptimum von 25mmHg durchschnittlich eingehalten werden. Demnach muss die Kompressionstherapie bezüglich des Alters nicht speziell angepasst werden. Ein signifikanter Unterschied kann zwischen neuen und alten Kompressionstherapien bestätigt werden.
Balanced hydroxyethylstarch (HES 130/0.4) impairs kidney function in-vivo without inflammation

Volume therapy is a standard procedure in daily perioperative care, and there is an ongoing discussion about the benefits of colloid resuscitation with hydroxyethylstarch (HES). In sepsis HES should be avoided due to a higher risk for acute kidney injury (AKI). Results of the usage of HES in patients without sepsis are controversial. Therefore we conducted an animal study to evaluate the impact of 6% HES 130/0.4 on kidney integrity with sepsis or under healthy conditions Sepsis was induced by standardized Colon Ascendens Stent Peritonitis (sCASP). sCASP-group as well as control group (C) remained untreated for 24 h. After 18 h sCASP+HES group (sCASP+VOL) and control+HES (C+VOL) received 50 ml/KG balanced 6% HES (VOL) 130/0.4 over 6h. After 24h kidney function was measured via Inulin- and PAH-Clearance in re-anesthetized rats, and serum urea, creatinine (crea), cystatin C and Neutrophil gelatinase-associated lipocalin (NGAL) as well as histopathology were analysed. In vitro human proximal tubule cells (PTC) were cultured +/- lipopolysaccharid (LPS) and with 0.1–4.0% VOL. Cell viability was measured with XTT-, cell toxicity with LDH-test. sCASP induced severe septic AKI demonstrated divergent results regarding renal function by clearance or creatinine measure focusing on VOL. Soleley HES (C+VOL) deteriorated renal function without sCASP. Histopathology revealed significantly derangements in all HES groups compared to control. In vitro LPS did not worsen the HES induced reduction of cell viability in PTC cells. For the first time, we demonstrated, that application of 50 ml/KG 6% HES 130/0.4 over 6 hours induced AKI without inflammation in vivo. Severity of sCASP induced septic AKI might be no longer susceptible to the way of volume expansion
Background
Solitary metastases to the pancreas are rare. Therefore the value of resection in curative intention remains unclear. In the literature there are several promising reports about resection of solitary metastasis to the pancreas mainly of renal origin.
Case presentation
Here we report for the first time on the surgical therapy of a 1.5 cm solitary pancreatic metastasis of an adrenocortical carcinoma. The metastasis occurred almost 6 years after resection of the primary tumor. A partial pancreatoduodenectomy was performed and postoperatively adjuvant mitotane treatment was initiated. During the follow-up of 3 years after surgery no evidence of tumor recurrence occurred.
Conclusion
Resection of pancreatic tumors should be considered, even if the mass is suspicious for metastatic disease including recurrence of adrenocortical cancer.
Background
Comprehensive evidence on the incidence, time course and independent risk factors of metachronous peritoneal carcinomatosis (metaPC) in gastric cancer patients treated with curative intent in the context of available systemic combination chemotherapies is lacking.
Methods
Data from a prospectively collected single-institutional Center Cancer Registry with 1108 consecutive patients with gastric adenocarcinoma (GC), clinical, histological and survival data were analyzed for independent risk factors and prognosis with focus on the development of metaPC. Findings were then stratified to the time periods of treatment with surgery alone, 5-Fluorouracil-only and contemporary combined systemic perioperative chemotherapy strategies, respectively.
Results
Despite R0 D2 gastrectomy (n = 560), 49.6% (±5.4%) of the patients were diagnosed with tumour recurrence and 15.5% (±1.8%) developed metaPC after a median time of 17.7 (15.1-20.3) months after surgery resulting in a tumour related mortality of 100% with a median survival of 3.0 months (2.1 – 4.0). Independent risk factors for the development of metaPC were serosa positive T-category, nodal positive-status, signet cell and undifferentiated gradings (G3/G4). Contemporary systemic combination chemotherapy did not improve the incidence and prognosis of metaPC (p = 0.54).
Conclusions
Despite significant improvements in the overall survival for the complete cohort with gastric cancer over time, those patients with metaPC did not experience the same benefits. The lack of change in the incidence, and persistent poor prognosis of metaPC after curative surgery expose the need for further prevention and/or improved treatment options for this devastating condition.
Immunity, Inflammation and Cancer: The role of Foxp3, TLR7 and TLR8 in gastrointestinal cancer

Regulatory T cells (Treg) expressing the transcription factor forkhead box protein P3 (Foxp3) have been demonstrated to mediate evasion from anti-tumor immune responses during tumor progression. Moreover, Foxp3 expression by tumor cells themselves may allow them to counteract effector T cell responses, resulting in a survival benefit of the tumor. For gastrointestinal cancers, in particular pancreatic and colorectal cancer (CRC), the clinical relevance of Foxp3 is not clear to date. Therefore the aim of this study was to analyze its impact in CRC and pancreatic cancer. To determine the relevance of Foxp3 for tumor progression and patient survival, gene and protein analysis of human pancreatic and colon cancer cell lines as well as tumor tissues from patients with CRC was performed. The results derived from the patients with CRC were correlated with clinicopathological parameters and patients’ overall survival. Cancer cell mediated Foxp3 expression in vitro was demonstrated in human pancreatic cancer cell lines PANC1, PaCa DD 135, PaCa DD 159 and PaCa DD 185 as well as in human colon cancer cell lines SW480 and SW620. Additionally, Foxp3 expressing cancer cells were found in ex vivo tumor tissue samples of patients with CRC. The percentage of Foxp3+ cancer cells increased from stages UICC I/II to UICC III/IV compared to normal tissue. Moreover, high tumor cell mediated Foxp3 expression was associated with poor prognosis compared to patients with low Foxp3 expression. In contrast, low and high Foxp3 level in tumor infiltrating Treg cells demonstrated no significant differences in patients’ overall survival. Correlation analysis demonstrated a significant association of Foxp3 cancer cell expression with the expression of immunosuppressive cytokines IL-10 and TGF-β. These findings suggest that Immunosuppressive cytokines such as IL-10 and TGF-β released by rather Foxp3+ cancer cells than Foxp3+ Treg cells may inhibit the activation of naive T cells, hence limiting antitumor immune responses and favoring tumorigenesis and progression.
Chronic inflammation has been shown to be an important epigenetic and environmental factor in numerous tumor entities. Recent data suggest that tumorigenesis and tumor progression may be associated with inflammation-triggered activation of Toll-like receptors (TLR). In this study, the specific impact of both TLR7 and TLR8 expression and signaling on tumor cell proliferation and chemoresistance is analyzed in inflammation linked CRC and pancreatic cancer. By gene and protein expression analysis of human pancreatic and colon cancer cell lines TLR7 and TLR8 expression was determined in vitro. Additionally, expression of TLR7/TLR8 in UICC stage I-IV pancreatic cancer, chronic pancreatitis and normal pancreatic tissue was examined. For in vitro/in vivo studies TLR7/TLR8 overexpressing PANC1 cell lines were generated and analyzed for effects of TLR expression and stimulation on tumor cell proliferation and chemoresistance. Cancer cell mediated TLR7 and TLR8 expression in vitro was demonstrated in human colon cancer cell lines SW480, SW620 and HT-29 as well as in primary pancreatic cancer cell lines PaCa DD 135, PaCa DD 159 and PaCa DD 185. Additionally, TLR7 and TLR8 expressing tumor cells were found in ex vivo tissue samples of patients with pancreatic cancer and chronic pancreatitis. Significantly elevated expression levels of TLR7 and TLR8 were found in advanced tumor stages (UICC III) compared to early tumor stages (UICC II) and chronic pancreatitis. No or occasionally low expression was detected in normal pancreatic tissue. In contrast to the tissues from patients with pancreatic cancer or chronic pancreatitis, established pancreatic tumor cell lines express only very low levels of TLR7 and TLR8. Therefore, for in vitro and xenograft studies TLR7 or TLR8 overexpressing PANC1 cells were generated. Proliferation promoting effects of TLR7 and TLR8 expression and stimulation with R848 were detected in vitro. Additionally, increased tumor growth of TLR expressing PANC1 cells was demonstrated in subcutaneously injected Balb/c nude mice. Interestingly, activation of TLR7 or TLR8 induced not only an increase in tumor cell proliferation but also a strong chemoresistance of PANC1 cells against 5-fluorouracil (5-FU). Moreover, treatment with R848 resulted in elevated expression levels of NF-κB, COX-2 and inflammatory cytokines IL-1β, IL-8 and TNF-α, suggesting TLR7/8 signaling to contribute to an inflammatory, anti-apoptotic and proliferation promoting tumor microenvironment. These findings emphasize the particular role of TLR7 and TLR8 in inflammation related cancers and their relevance as potential targets for cancer therapy.
