Durch einen auffällig hohen Anteil an Patienten mit Autoimmungastritis, die gleichzeitig Antikörper gegen Helicobacter pylori aufweisen, wurde eine enge pathogenetische Korrellation der beiden Krankheitsbilder vermutet. So macht eine Entstehungstheorie der Autoimmungastritis eine Supermutation der B-Zellen nach Antigenkontakt mit dem H.pylori für eine autoimmune Entgleisung dieser verantwortlich. Die molekulargenetischen Untersuchungen der antikörperkodierenden Gene und deren Mutationen zeigen jedoch, daß das Mutationsniveau in der Autoimmungastritis niedriger ist als dies in der H. pylori Gastritis. Die Theorie der durch Supermutationen entstehenden autoimmungastritis aus der H.pylori Gastritis konnte in dieser Arbeit nicht bestätigt werden. Auch wurde durch des Aufstellen genealogischer B-Zellklon Stammbäume eine Kompartimentierung der inflammatorischen Zellen nach anatomischen Regionen, wie es das histopathologisches Bild der beschriebenen Gastritiden vermuten ließen nicht darstellen.
We herein report the case of a 73‐year‐old male patient who was diagnosed with leukemic non‐nodal MCL. This patient had received six cycles of bendamustine, which resulted in a transient remission, and a second‐line therapy with ibrutinib, which unfortunately failed to induce remission. We started a treatment with single‐agent obinutuzumab at a dose of 20 mg on day 1, 50 mg on day 2‐4, 330 mg on day 5, and 1000 mg on day 6. The laboratory analysis showed a rapid decrease of leukocyte count. Four weeks later, we repeated the treatment with obinutuzumab at a dose of 1000 mg q4w and started a therapy with venetoclax at a dose of 400 mg qd, which could be increased to 800 mg qd from the third cycle. This combination therapy was well tolerated. The patient achieved a complete remission (CR) after three cycles of obinutuzumab and venetoclax. To date, the patient has a progression‐free survival of 17 months under ongoing obinutuzumab maintenance q4w. This is the first report about obinutuzumab and venetoclax induced CR in rituximab‐intolerant patient with an ibrutinib‐resistant MCL. This case suggests that obinutuzumab‐ and venetoclax‐based combination therapy might be salvage therapy in patients with ibrutinib‐resistant MCL.
Das kolorektale Karzinom stellt die dritthäufigste Tumorerkrankung weltweit dar. Die Risikofaktoren sind vielseitig und werden in exogene und endogene Faktoren eingeteilt. Eine wichtige Präventionsmaßnahme von Kolonkarzinom ist die komplette endoskopische Koloskopie, die ab dem 55. Lebensjahr empfohlen wird. Der Goldstandard zur Behandlung von Kolonkarzinom ist nach wie vor die chirurgische Tumorresektion mit mikroskopisch nachgewiesener Tumorfreiheit. Eine chirurgische Sanierung der Fernmetastasen, welche am häufigsten in der Leber vorkommen, ist bei betroffenen Patienten anzustreben. Eine adjuvante Chemotherapie wird je nach UICC-Stadium des Tumors durchgeführt. Im Gegensatz zur Behandlung einiger maligner Tumorerkrankungen ist der Einsatz von Antikörpern noch kein fester Bestandteil der Therapie von Kolonkarzinomen.
In dieser Arbeit wurde Untersuchungsmaterial von 41 Patienten mit Kolonkarzinom, die am Universitätsklinikum Würzburg in den Jahren 1997 bis 2012 behandelt wurden, analysiert. Dabei wurden Paraffinschnitte vom Primärtumor, regionalen Lymphknotenmetastasen und Lebermetastasen der einzelnen Patienten mit 2 verschiedenen monoklonalen IgM-Antikörpern, PAT-SM6 und PAT-LM1, gefärbt und mikroskopisch untersucht. Der Antikörper PAT-SM6 wurde aus einem an einem Magenkarzinom erkrankten Patienten isoliert und bindet an eine Isotyp-Form des 'Glucose-Regulated' Protein (GRP)-78PAT-SM6. Als Zielstruktur des PAT-LM1 Antikörpers wurde eine tumorspezifische Form von NONO (Non-POU domain-containing octamer-binding protein) identifiziert (NONOPAT-LM1). Für beide Rezeptor-Isoformen wurde nachgewiesen, dass sie nur auf malignen epithelialen Zellen, nicht aber auf gesunden Zellen exprimiert werden. Anhand dieser Arbeit konnte gezeigt werden, dass PAT-SM6 die Tumorzellen der Lebermetastasen stärker anfärbte als Zellen des Primärtumors. Für die PAT-LM1 Antikörperfärbung wurde ein ähnliches Resultat erzielt. In Bezug auf das Lebensalter der Patienten wiesen die Tumorzellen von älteren Patienten (ab dem 65. Lebensjahr) eine stärkere Antikörperbindung durch PAT-SM6 und PAT-LM1 auf. Interessant war auch die Feststellung, dass die Tumorzellen der Lebermetastasen von verstorbenen Patienten durch PAT-LM1 stärker gefärbt waren als die von zum Untersuchungszeitpunkt noch lebenden Patienten. Die Bindungsunterschiede zwischen PAT-SM6 und PAT-LM1 könnten neue diagnostische und therapeutische Möglichkeiten bei Kolonkarzinomen bieten und somit zukünftig eine individuelle Tumortherapie ermöglichen.
The subclassification of diffuse large B-cell lymphoma (DLBCL) into germinal center B-cell-like (GCB) and activated B-cell-like (ABC) subtypes has become mandatory in the 2017 update of the WHO classification of lymphoid neoplasms and will continue to be used in the WHO 5\(^{th}\) edition. The RNA-based Lymph2Cx assay has been validated as a reliable surrogate of high-throughput gene expression profiling assays for distinguishing between GCB and ABC DLBCL and provides reliable results from formalin-fixed, paraffin-embedded (FFPE) material. This test has been previously used in clinical trials, but experience from real-world routine application is rare. We routinely applied the Lymph2Cx assay to day-to-day diagnostics on a series of 147 aggressive B-cell lymphoma cases and correlated our results with the immunohistochemical subclassification using the Hans algorithm and fluorescence in situ hybridization findings using break-apart probes for MYC, BCL2, and BCL6. The routine use of the Lymph2Cx assay had a high technical success rate (94.6%) with a low rate of failure due to poor material and/or RNA quality. The Lymph2Cx assay was discordant with the Hans algorithm in 18% (23 of 128 cases). Discordant cases were mainly classified as GCB by the Hans algorithm and as ABC by Lymph2Cx (n = 11, 8.6%). Only 5 cases (3.9%) were classified as non-GCB by the Hans algorithm and as GCB by Lymph2Cx. Additionally, 5.5% of cases (n = 7) were left unclassified by Lymph2Cx, whereas they were defined as GCB (n = 4) or non-GCB (n = 3) by the Hans algorithm. Our data support the routine applicability of the Lymph2Cx assay.
In this thesis we have investigated the effect of NFAT (Nuclear Factor of Activated T Cell) transcription factors on the expression of Rag-(Recombination Activating Genes) genes in murine thymus. The protein products of Rag genes, RAG1 and RAG2, are critical for the recombination and generation of the TCR (T Cell Receptor) repertoire during thymocyte development, and their expression can be suppressed by the activity of NFAT factors. In thymus, the expression of Rag1 and Rag2 genes is induced at the double-negative (DN, CD4-8-) 3 stage, down-regulated at the DN4 stage, re-induced at the double-positive (DP, CD4+8+) stage, and suppressed again at the single-positive (SP, CD4+8- or CD4-8+) stage. Although it is known that TCR signaling suppresses the expression of Rag1 and Rag2 at the SP stage, the signals that mediate the Rag gene down-reulation remain elusive. Here we report that both the calcineurin-NFAT-signaling and MAPKinase signaling pathways, which are activated by TCR signaling during positive selection, mediate the Rag gene down-regulation in DP thymocytes. The calcineurin-NFAT pathway suppresses both the Rag1 and the Rag2 gene expression. This pathway has a stronger suppressive effect on the Rag1 than the Rag2 gene. A synergistic activity between the two NFAT factors NFATc2 and NFATc3 is essential for calcineurin-NFAT signaling to efficiently suppress the Rag gene expression in DP thymocytes. It is likely that the calcineurin-NFAT signaling down-regulates Rag gene expression by suppressing both the Rag anti-silencer element (ASE) activity and the Rag promoter activity. Similarly, MEK-ERK signaling of MAPK signaling pathway mediates the Rag gene suppression in DP thymocytes although the mechanism through which MEK-ERK mediates the Rag gene down-regulation has to be elucidated. In DN thymocytes, it appears that neither the calcineurin-NFAT signaling nor MAPK signaling is involved in the Rag gene down-regulation. However, a role for these two signaling pathways in the Rag gene up-regulation in DN thymocytes is not excluded. In DN thymocytes, pre-TCR signaling stimulates the expression both Nfatc1 and Nfatc2 genes but has no effect on Nfatc3 gene expression. In DN thymocytes, pre-TCR signaling activates Nfatc1α expression but not Nfatc1ß expression, i.e. the two promoters controling Nfatc1 gene xpression are differently controled by pre-TCR signals. Nfatc1α gene expression in DN thymocytes is mainly regulated by the MAPK signaling pathway because activation of Nfatc1α is mediated by MEK-ERK signaling but opposed by JNK signaling. Calcineuirn-NFAT and p38 signaling pathways are not involved in Nfatc1α promoter regulation in DN thymocytes. In DP thymocytes, TCR signaling up-regulates Nfatc1 and Nfatc2 expression but down-regulates Nfatc3 expression. In DP thymocytes, TCR signaling activates Nfatc1α expression. The activation of Nfatc1α in DP thymocytes is mediated by NFATc1, but not or to a less degree by NFATc2 and NFATc3. MEK-ERK, JNK, and p38 signaling pathways are involved in Nfatc1α gene activation in DP thymocytes, probably by activating NFAT trans-activation activity. All these findings illustrate that in thymocytes the expression of NFAT transcription factors – which are essential for thymic development - is controled at multiple levels.
CD9 is the best-studied member of the tetraspanin family of transmembrane proteins. It is involved in various fundamental cellular processes and its altered expression is a characteristic of malignant cells of different origins. Despite numerous investigations confirming its fundamental role, the heterogeneity of CD9 or other tetraspanin proteins was considered only to be caused by posttranslational modification, rather than alternative splicing. Here we describe the first identification of CD9 transcript variants expressed by cell lines derived from fetal rat brain cells. Variant mRNA-B lacks a potential translation initiation codon in the alternative exon 1 and seems to be characteristic of the tumorigenic BT cell lines. In contrast, variant mRNA-C can be translated from a functional initiation codon located in its extended exon 2, and substantial amounts of this form detected in various tissues suggest a contribution to CD9 functions. From the alternative sequence of variant C, a different membrane topology ( 5 transmembrane domains) and a deviating spectrum of functions can be expected.
Background: Primary cutaneous follicular B-cell lymphoma (PCFBCL) represents an indolent subtype of Non-Hodgkin’s lymphomas, being clinically characterized by slowly growing tumors of the skin and common cutaneous relapses, while only exhibiting a low propensity for systemic dissemination or fatal outcome. Up to now, only few studies have investigated underlying molecular alterations of PCFBCL with respect to somatic mutations. Objectives: Our aim was to gain deeper insight into the pathogenesis of PCFBCL and to delineate discriminatory molecular features of this lymphoma subtype. Methods: We performed hybridization-based panel sequencing of 40 lymphoma-associated genes of 10 cases of well-characterized PCFBCL. In addition, we included two further ambiguous cases of atypical B-cell-rich lymphoid infiltrate/B-cell lymphoma of the skin for which definite subtype attribution had not been possible by routine investigations. Results: In 10 out of 12 analyzed cases, we identified genetic alterations within 15 of the selected 40 target genes. The most frequently detected alterations in PCFBCL affected the TNFRSF14, CREBBP, STAT6 and TP53 genes. Our analysis unrevealed novel mutations of the BCL2 gene in PCFBCL. All patients exhibited an indolent clinical course. Both the included arbitrary cases of atypical B-cell-rich cutaneous infiltrates showed somatic mutations within the FAS gene. As these mutations have previously been designated as subtype-specific recurrent alterations in primary cutaneous marginal zone lymphoma (PCMZL), we finally favored the diagnosis of PCMZL in these two cases based on these molecular findings. Conclusions: To conclude, our molecular data support that PCFBCL shows distinct somatic mutations which may aid to differentiate PCFBCL from pseudo-lymphoma as well as from other indolent and aggressive cutaneous B-cell lymphomas. While the detected genetic alterations of PCFBCL did not turn out to harbor any prognostic value in our cohort, our molecular data may add adjunctive discriminatory features for diagnostic purposes on a molecular level.
Introduction: Large-cell transformation (LCT) of mycosis fungoides (MF) has been associated with a higher risk of relapse and progression and, consequently, restricted prognosis. Its molecular pathogenesis has not been elucidated yet. Materials and Methods: In order to address molecular mechanisms of LCT, we performed hybrid capture panel-based sequencing of skin biopsies from 10 patients suffering from MF with LCT versus 17 patients without LCT including follow-up biopsies during clinical course, respectively (51 samples in total). The analyzed patients were attributed to three different groups based on the presence of LCT and clinical behavior. Results: While indolent MF cases without LCT did not show pathogenic driver mutations, a high rate of oncogenic alterations was detected in patients with LCT and aggressive clinical courses. Various genes of different oncogenic signaling pathways, including the MAPK and JAK-STAT signaling pathways, as well as epigenetic modifiers were affected. A high inter-individual and distinctive intra-individual mutation diversity was observed. Oncogenic RAS mutations were exclusively detected in patients with LCT. Conclusion: Our data demonstrate that LCT transition of MF is associated with increased frequency of somatic mutations in cancer-associated genes. In particular, the activation of RAS signaling — together with epigenetic dysregulation — may crucially contribute to the molecular pathogenesis of the LCT phenotype, thus conveying its adverse clinical behavior.
Das follikuläre Lymphom (FL) wird nach der aktuellen Klassifikation der WHO (World Health Organization Classification of Lymphoid Tumours) anhand der Zahl der Zentroblasten in drei Grade und der Grad 3 weiter in 3A und 3B eingeteilt. Bis heute ist die Rolle der FL3B aufgrund der morphologischen und genetischen Unterschiede zu den anderen FL umstritten, es wird eine eigene Entität und Pathogenese des FL3B diskutiert. Durch das Verbundprojekt „Molekulare Mechanismen in malignen Lymphomen“ (MMML) Daten zu FISH-, Genexpressionsanalysen und immunhistochemischen Färbungen bearbeitet werden.
Diesen Daten zufolge sind FL3B in ihrer Genexpression nicht von FL3A trennbar. Es konnte jedoch eine Abgrenzung der FL1/2 zu den FL3A/B durch die erhöhte Expression von 13 Genen in den FL3A/B gefunden werden, von denen Homolog, double strand break repair nuclease (MRE11A), Topoisomerase II alpha (TOP2A) und Thioredoxin (TXN) schon zuvor im Rahmen von FL und NHL diskutiert wurden.
Der humane monoklonale IgM-Antikörper PAM-1 induziert in seiner monomeren Form sowohl in vivo als auch in vitro Apoptose an Magenkarzinomzellen. Er bindet hierbei an den post-transskriptionell modifizierten Rezeptor CFR-1, der auf nahezu allen epithelialen Karzinomzellen und deren Vorläuferläsionen exprimiert wird. In dieser Arbeit werden die intrazellulären Mechanismen nach PAM-1/CFR-1-Bin- dung anhand von Protein(de)phosphorylierungen und deren selektiver Hemmung näher charakterisiert. Die hierbei detektierten Protein(de)phosphorylierungen sind somit möglicherweise essentielle Bestandteile der durch PAM-1 ausgelösten Apoptose.
The Proteome Profiles of the Cerebellum of Juvenile, Adult and Aged Rats-An Ontogenetic Study

In this study, we searched for proteins that change their expression in the cerebellum (Ce) of rats during ontogenesis. This study focuses on the question of whether specific proteins exist which are differentially expressed with regard to postnatal stages of development. A better characterization of the microenvironment and its development may result from these study findings. A differential two-dimensional polyacrylamide gel electrophoresis (2DE) and matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF-MS) analysis of the samples revealed that the number of proteins of the functional classes differed depending on the developmental stages. Especially members of the functional classes of biosynthesis, regulatory proteins, chaperones and structural proteins show the highest differential expression within the analyzed stages of development. Therefore, members of these functional protein groups seem to be involved in the development and differentiation of the Ce within the analyzed development stages. In this study, changes in the expression of proteins in the Ce at different postnatal developmental stages (postnatal days (P) 7, 90, and 637) could be observed. At the same time, an identification of proteins which are involved in cell migration and differentiation was possible. Especially proteins involved in processes of the biosynthesis and regulation, the dynamic organization of the cytoskeleton as well as chaperones showed a high amount of differentially expressed proteins between the analyzed dates.
The proteome profiles of the olfactory bulb of juvenile, adult and aged rats - an ontogenetic study

Background:
In this study, we searched for proteins that change their expression in the olfactory bulb (oB) of rats during ontogenesis. Up to now, protein expression differences in the developing animal are not fully understood. Our investigation focused on the question whether specific proteins exist which are only expressed during different development stages. This might lead to a better characterization of the microenvironment and to a better determination of factors and candidates that influence the differentiation of neuronal progenitor cells.
Results:
After analyzing the samples by two-dimensional polyacrylamide gel electrophoresis (2DE) and matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF-MS), it could be shown that the number of expressed proteins differs depending on the developmental stages. Especially members of the functional classes, like proteins of biosynthesis, regulatory proteins and structural proteins, show the highest differential expression in the stages of development analyzed.
Conclusion:
In this study, quantitative changes in the expression of proteins in the oB at different developmental stages (postnatal days (P) 7, 90 and 637) could be observed. Furthermore, the expression of many proteins was found at specific developmental stages. It was possible to identify these proteins which are involved in processes like support of cell migration and differentiation.
Current guidelines recommend consolidation with autologous stem cell transplantation (autoSCT) after induction chemotherapy for most patients with peripheral T-cell lymphoma (PTCL). This assumption is based on five prospective phase II studies, three of which included <50 patients with limited follow-up. Here we present the final analysis of the prospective German study. The treatment regimen consisted of four to six cycles of CHOP chemotherapy followed by mobilizing therapy and stem cell collection. Patients in complete remission (CR) or partial remission (PR) underwent myeloablative chemo(radio)therapy and autoSCT. From January 2001 to July 2010, 111 patients were enrolled in the study. The main subgroups were PTCL not specified (n=42) and angioimmunoblastic T-cell lymphoma (n=37). Seventy-five (68%) of the 111 patients received transplantation. The main reason for not receiving autoSCT was progressive disease. In an intent-to-treat analysis, the complete response rate after myeloablative therapy was 59%. The estimated 5-year overall survival, disease-free survival and progression-free survival rates were 44%, 54% and 39%, respectively. The results of this study confirm that upfront autoSCT can result in long-term remissions in patients with all major subtypes of PTCL and therefore should be part of first-line therapy whenever possible.
Disclosing the CXCR4 expression in lymphoproliferative diseases by targeted molecular imaging

Chemokine ligand-receptor interactions play a pivotal role in cell attraction and cellular trafficking, both in normal tissue homeostasis and in disease. In cancer, chemokine receptor-4 (CXCR4) expression is an adverse prognostic factor. Early clinical studies suggest that targeting CXCR4 with suitable high-affinity antagonists might be a novel means for therapy. In addition to the preclinical evaluation of [\(^{68}\)Ga]Pentixafor in mice bearing human lymphoma xenografts as an exemplary CXCR4-expressing tumor entity, we report on the first clinical applications of [\(^{68}\)Ga]Pentixafor-Positron Emission Tomography as a powerful method for CXCR4 imaging in cancer patients. [\(^{68}\)Ga]Pentixafor binds with high affinity and selectivity to human CXCR4 and exhibits a favorable dosimetry. [\(^{68}\)Ga]Pentixafor-PET provides images with excellent specificity and contrast. This non-invasive imaging technology for quantitative assessment of CXCR4 expression allows to further elucidate the role of CXCR4/CXCL12 ligand interaction in the pathogenesis and treatment of cancer, cardiovascular diseases and autoimmune and inflammatory disorders.
C-X-C motif chemokine receptor 4 (CXCR4) and somatostatin receptors (SSTR) are overexpressed in gastro-entero-pancreatic neuroendocrine tumors (GEP-NET). In this study, we aimed to elucidate the feasibility of non-invasive CXCR4 positron emission tomography/computed tomography (PET/CT) imaging in GEP-NET patients using [\(^{68}\)Ga]Pentixafor in comparison to \(^{68}\)Ga-DOTA-D-Phe-Tyr3-octreotide ([\(^{68}\)Ga]DOTATOC) and \(^{18}\)F-fluorodeoxyglucose ([\(^{18}\)F]FDG). Twelve patients with histologically proven GEP-NET (3xG1, 4xG2, 5xG3) underwent [\(^{68}\)Ga]DOTATOC, [\(^{18}\)F]FDG, and [\(^{68}\)Ga]Pentixafor PET/CT for staging and planning of the therapeutic management. Scans were analyzed on a patient as well as on a lesion basis and compared to immunohistochemical staining patterns of CXCR4 and somatostatin receptors SSTR2a and SSTR5. [\(^{68}\)Ga]Pentixafor visualized tumor lesions in 6/12 subjects, whereas [\(^{18}\)F]FDG revealed sites of disease in 10/12 and [\(^{68}\)Ga]DOTATOC in 11/12 patients, respectively. Regarding sensitivity, SSTR-directed PET was the superior imaging modality in all G1 and G2 NET. CXCR4-directed PET was negative in all G1 NET. In contrast, 50% of G2 and 80% of G3 patients exhibited [\(^{68}\)Ga]Pentixafor-positive tumor lesions. Whereas CXCR4 seems to play only a limited role in detecting well-differentiated NET, increasing receptor expression could be non-invasively observed with increasing tumor grade. Thus, [\(^{68}\)Ga]Pentixafor PET/CT might serve as non-invasive read-out for evaluating the possibility of CXCR4-directed endoradiotherapy in advanced dedifferentiated SSTR-negative tumors.
Exon-4 Mutations in KRAS Affect MEK/ERK and PI3K/AKT Signaling in Human Multiple Myeloma Cell Lines

Approximately 20% of multiple myeloma (MM) cases harbor a point mutation in KRAS. However, there is still no final consent on whether KRAS-mutations are associated with disease outcome. Specifically, no data exist on whether KRAS-mutations have an impact on survival of MM patients at diagnosis in the era of novel agents. Direct blockade of KRAS for therapeutic purposes is mostly impossible, but recently a mutation-specific covalent inhibitor targeting KRAS\(^{p.G12C}\) entered into clinical trials. However, other KRAS hotspot-mutations exist in MM patients, including the less common exon-4 mutations. For the current study, the coding regions of KRAS were deep-sequenced in 80 newly diagnosed MM patients, uniformely treated with three cycles of bortezomib plus dexamethasone and cyclophosphamide (VCD)-induction, followed by high-dose chemotherapy and autologous stem cell transplantation. Moreover, the functional impact of KRAS\(^{p.G12A}\) and the exon-4 mutations p.A146T and p.A146V on different survival pathways was investigated. Specifically, KRAS\(^{WT}\), KRAS\(^{p.G12A}\), KRAS\(^{p.A146T}\), and KRAS\(^{p.A146V}\) were overexpressed in HEK293 cells and the KRAS\(^{WT}\) MM cell lines JJN3 and OPM2 using lentiviral transduction and the Sleeping Beauty vector system. Even though KRAS-mutations were not correlated with survival, all KRAS-mutants were found capable of potentially activating MEK/ERK- and sustaining PI3K/AKT-signaling in MM cells.
The treatment of Parkinson's disease by transplantation of dopaminergic (DA) neurons from human embryonic mesencephalic tissue is a promising approach. However, the origin of these cells causes major problems: availability and standardization of the graft. Therefore, the generation of unlimited numbers of DA neurons from various types of stem or progenitor cells has been brought into focus. A source for DA neurons might be conditionally immortalized progenitor cells. The temperature-sensitive immortalized cell line CSM14.1 derived from the mesencephalon of an embryonic rat has been used successfully for transplantation experiments. This cell line was analyzed by unbiased stereology of cell type specific marker proteins and 2D-gel electrophoresis followed by mass spectrometry to characterize the differentially expressed proteome. Undifferentiated CSM14.1 cells only expressed the stem cell marker nestin, whereas differentiated cells expressed GFAP or NeuN and tyrosine hydroxylase. An increase of the latter cells during differentiation could be shown. By using proteomics an explanation on the protein level was found for the observed changes in cell morphology during differentiation, when CSM14.1 cells possessed the morphology of multipolar neurons. The results obtained in this study confirm the suitability of CSM14.1 cells as an in vitro model for the study of neuronal and dopaminergic differentiation in rats.
Somatic mutations in protein kinase A catalytic α subunit (PRKACA) were found to be causative for 30-40% of cortisol-producing adenomas (CPA) of the adrenal gland, rendering PKA signalling constitutively active. In its resting state, PKA is a stable and inactive heterotetramer, consisting of two catalytic and two regulatory subunits with the latter inhibiting PKA activity. The human genome encodes three different PKA catalytic subunits and four different regulatory subunits that are preferentially expressed in different organs. In normal adrenal glands all regulatory subunits are expressed, while CPA exhibit reduced protein levels of the regulatory subunit IIβ. In this study, we linked for the first time the loss of RIIβ protein levels to the PRKACA mutation status and found the down-regulation of RIIβ to arise post-transcriptionally. We further found the PKA subunit expression pattern of different tumours is also present in the zones of the normal adrenal cortex and demonstrate that the different PKA subunits have a differential expression pattern in each zone of the normal adrenal gland, indicating potential specific roles of these subunits in the regulation of different hormones secretion.
We aimed to elucidate the diagnostic potential of the C-X-C motif chemokine receptor 4 (CXCR4)-directed positron emission tomography (PET) tracer \(^{68}\)Ga-Pentixafor in patients with poorly differentiated neuroendocrine carcinomas (NEC), relative to the established reference standard \(^{18}\)F-FDG PET/computed tomography (CT). In our database, we retrospectively identified 11 treatment-naïve patients with histologically proven NEC, who underwent \(^{18}\)F-FDG and CXCR4-directed PET/CT for staging and therapy planning. The images were analyzed on a per-patient and per-lesion basis and compared to immunohistochemical staining (IHC) of CXCR4 from PET-guided biopsies. \(^{68}\)Ga-Pentixafor visualized tumor lesions in 10/11 subjects, while \(^{18}\)F-FDG revealed sites of disease in all 11 patients. Although weak to moderate CXCR4 expression could be corroborated by IHC in 10/11 cases, \(^{18}\)F-FDG PET/CT detected significantly more tumor lesions (102 vs. 42; total lesions, n = 107; p < 0.001). Semi-quantitative analysis revealed markedly higher 18F-FDG uptake as compared to \(^{68}\)Ga-Pentixafor (maximum and mean standardized uptake values (SUV) and tumor-to-background ratios (TBR) of cancerous lesions, SUVmax: 12.8 ± 9.8 vs. 5.2 ± 3.7; SUVmean: 7.4 ± 5.4 vs. 3.1 ± 3.2, p < 0.001; and, TBR 7.2 ± 7.9 vs. 3.4 ± 3.0, p < 0.001). Non-invasive imaging of CXCR4 expression in NEC is inferior to the reference standard \(^{18}\)F-FDG PET/CT.
TP53 mutations have been associated with anaplasia in Wilms tumour, which conveys a high risk for relapse and fatal outcome. Nevertheless, TP53 alterations have been reported in no more than 60% of anaplastic tumours, and recent data have suggested their presence in tumours that do not fulfil the criteria for anaplasia, questioning the clinical utility of TP53 analysis. Therefore, we characterized the TP53 status in 84 fatal cases of Wilms tumour, irrespective of histological subtype. We identified TP53 alterations in at least 90% of fatal cases of anaplastic Wilms tumour, and even more when diffuse anaplasia was present, indicating a very strong if not absolute coupling between anaplasia and deregulation of p53 function. Unfortunately, TP53 mutations do not provide additional predictive value in anaplastic tumours since the same mutation rate was found in a cohort of non-fatal anaplastic tumours. When classified according to tumour stage, patients with stage I diffuse anaplastic tumours still had a high chance of survival (87%), but this rate dropped to 26% for stages II–IV. Thus, volume of anaplasia or possible spread may turn out to be critical parameters. Importantly, among non-anaplastic fatal tumours, 26% had TP53 alterations, indicating that TP53 screening may identify additional cases at risk. Several of these non-anaplastic tumours fulfilled some criteria for anaplasia, for example nuclear unrest, suggesting that such partial phenotypes should be under special scrutiny to enhance detection of high-risk tumours via TP53 screening. A major drawback is that these alterations are secondary changes that occur only later in tumour development, leading to striking intratumour heterogeneity that requires multiple biopsies and analysis guided by histological criteria. In conclusion, we found a very close correlation between histological signs of anaplasia and TP53 alterations. The latter may precede development of anaplasia and thereby provide diagnostic value pointing towards aggressive disease.
The introduction of new therapeutic agents has revolutionized the treatment of metastatic melanoma. The approval of adjuvant anti‐programmed death‐1 monotherapy with nivolumab or pembrolizumab, and dabrafenib plus trametinib has recently set a new landmark in the treatment of stage III melanoma. Now, clinical trials have shown that immune checkpoint blockade can be performed in a neoadjuvant setting, an approach established as a standard therapeutic approach for other tumour entities such as breast cancer. Recent studies suggest that a pathological response achieved by neoadjuvant immunotherapy is associated with long‐term tumour control and that short neoadjuvant application of checkpoint inhibitors may be superior to adjuvant therapy. Most recently, neoadjuvant ipilimumab plus nivolumab in stage III melanoma was reported. With two courses of dose‐optimized ipilimumab (1 mg kg−1) combined with nivolumab (3 mg kg−1), pathological responses were observed in 77% of patients, while only 20% of patients experienced grade 3 or 4 adverse events. However, the neoadjuvant trials employing combined immune checkpoint blockade conducted so far have excluded patients with in transit metastases, a common finding in stage III melanoma. Here we report four patients with in transit metastases or an advanced primary tumour who have been treated with neoadjuvant ipilimumab plus nivolumab according to the OpACIN‐neo trial scheme (arm B). All patients achieved radiological disease control and a pathological response. None of the patients has relapsed so far.
Due to the rapidly increasing development and use of cellular products, there is a rising demand for non-animal-based test platforms to predict, study and treat undesired immunity. Here, we generated human organotypic skin models from human biopsies by isolating and expanding keratinocytes, fibroblasts and microvascular endothelial cells and seeding these components on a collagen matrix or a biological vascularized scaffold matrix in a bioreactor. We then were able to induce inflammation-mediated tissue damage by adding pre-stimulated, mismatched allogeneic lymphocytes and/or inflammatory cytokine-containing supernatants histomorphologically mimicking severe graft versus host disease (GvHD) of the skin. This could be prevented by the addition of immunosuppressants to the models. Consequently, these models harbor a promising potential to serve as a test platform for the prediction, prevention and treatment of GvHD. They also allow functional studies of immune effectors and suppressors including but not limited to allodepleted lymphocytes, gamma-delta T cells, regulatory T cells and mesenchymal stromal cells, which would otherwise be limited to animal models. Thus, the current test platform, developed with the limitation that no professional antigen presenting cells are in place, could greatly reduce animal testing for investigation of novel immune therapies.
Epstein-Barr virus (EBV) is best known for infection of B cells, in which it usually establishes an asymptomatic lifelong infection, but is also associated with the development of multiple B cell lymphomas. EBV also infects epithelial cells and is associated with all cases of undifferentiated nasopharyngeal carcinoma (NPC). EBV is etiologically linked with at least 8% of gastric cancer (EBVaGC) that comprises a genetically and epigenetically distinct subset of GC. Although we have a very good understanding of B cell entry and lymphomagenesis, the sequence of events leading to EBVaGC remains poorly understood. Recently, ephrin receptor A2 (EPHA2) was proposed as the epithelial cell receptor on human cancer cell lines. Although we confirm some of these results, we demonstrate that EBV does not infect healthy adult stem cell-derived gastric organoids. In matched pairs of normal and cancer-derived organoids from the same patient, EBV only reproducibly infected the cancer organoids. While there was no clear pattern of differential expression between normal and cancer organoids for EPHA2 at the RNA and protein level, the subcellular location of the protein differed markedly. Confocal microscopy showed EPHA2 localization at the cell-cell junctions in primary cells, but not in cancer cell lines. Furthermore, histologic analysis of patient tissue revealed the absence of EBV in healthy epithelium and presence of EBV in epithelial cells from inflamed tissue. These data suggest that the EPHA2 receptor is not accessible to EBV on healthy gastric epithelial cells with intact cell-cell contacts, but either this or another, yet to be identified receptor may become accessible following cellular changes induced by inflammation or transformation, rendering changes in the cellular architecture an essential prerequisite to EBV infection.
Genetische Veränderungen von Thymomen, insbesondere rekurrente Aberrationen, sind bislang unbekannt. In dieser Dissertation wurden 13 WHO Typ A Thymome, 23 WHO Typ B3 Thymome sowie 12 primäre Plattenepithelkarzinome (WHO Typ C Thymome) sowie einige seltene Primärtumoren des Thymus mittels Komparativer genomischer Hybridisierung untersucht. Mit Ausnahme eines einzigen Falles zeigten WHO Typ A Thymome keine chromosomalen Gewinne oder Verluste. Im Gegensatz dazu fanden sich bei allen Thymomen des Typs B3 genetische Aberrationen. Der Gewinn von 1q trat in 15 (65%), Gewinne von Chromosom 16 und Xq traten in jeweils 3 (13%) Fällen auf. Verluste fanden sich auf Chromosom 6 in 10 Fällen (43%) und an 13q in 6 Fällen (23%). Die häufigsten Gewinne bei primären Plattenepithelkarzinomen fanden sich auf 1q in 7 Fällen (58%), auf 17q in 4 Fällen (33%) und auf Chromosom 5 und 18 in jeweils 3 Fällen (25%). Rekurrente Verluste traten bei dieser Entität auf Chromosom 16q in 8 Fällen (67%), auf Chromosom 6 in 6 Fällen (50%), auf Chromosom 3 in 4 Fällen (33%) und auf den Chromosomen 13q sowie 17p in jeweils 3 Fällen (25%) auf. Die Dissertation zeigt, daß histologisch unterschiedliche Thymome mit unterschiedlichen, rekurrenten Aberrationen assoziiert sind. WHO Typ A Thymome weisen nur vereinzelt genetische Aberrationen auf. Thymome vom WHO Typ A und Typ B3 weisen verschiedene genetische Phänotypen auf, während Thymome vom Typ B3 sowie die primären Plattenepithelkarzinome des Thymus gemeinsame genetische Aberrationen zeigen. Neben den pathogenetischen Aspekten weist der beobachtete Verlust von Chromosom 6, auf dem sich die Genloci der HLA-Moleküle befinden, bei WHO Typ B3 Thymomen auch auf eine Rolle bei der Entstehung paraneoplastischer Autoimmunphänomene wie der Myasthenia Gravis hin.
Background:
Conventional parameters including Ki67, hormone receptor and Her2/neu status are used for risk stratification for breast cancer. The serine protease urokinase plasminogen activator (uPA) and the plasminogen activator inhibitor type-1 (PAI-1) play an important role in tumour invasion and metastasis. Increased concentrations in tumour tissue are associated with more aggressive potential of the disease. Multigene tests provide detailed insights into tumour biology by simultaneously testing several prognostically relevant genes. With OncotypeDX\(^{®}\), a panel of 21 genes is tested by means of quantitative real-time polymerase chain reaction.
The purpose of this pilot study was to analyse whether a combination of Ki67 and uPA/PAI-1 supplies indications of the result of the multigene test.
Methods:
The results of Ki67, uPA/PAI-1 and OncotypeDX\(^{®}\) were analysed in 25 breast carcinomas (luminal type, pT1/2, max pN1a, G2). A statistical and descriptive analysis was performed.
Results:
With a proliferation index Ki67 of < 14%, the recurrence score (RS) from the multigene test was on average in the low risk range, with an intermediate RS usually resulting if Ki67 was > 14%. Not elevated values of uPA and PAI-1 showed a lower rate of proliferation (average 8.5%) than carcinomas with an increase of uPA and/or PAI-1 (average 13.9%); p = 0.054, Student’s t-test. When Ki67 was > 14% and uPA and/or PAI-1 was raised, an intermediate RS resulted. These differences were significant when compared to cases with Ki67 < 14% with non-raised uPA/PAI-1 (p < 0.03, Student’s t-test). Without taking into account the proliferative activity, an intermediate RS was also verifiable if both uPA and PAI-1 showed raised values.
Conclusion:
A combination of the values Ki67 and uPA/PAI-1 tended to depict the RS to be expected. From this it can be deduced that an appropriate analysis of this parameter combination may be undertaken before the multigene test in routine clinical practice. The increasing cost pressure makes it necessary to base the implementation of a multigene test on ancillary variables and to potentially leave it out if not required in the event of a certain constellation of results (Ki67 raised, uPA and PAI-1 raised).
Die Unterscheidung zwischen körpereigenen und körperfremden Strukturen ist eine grundlegende Herausforderung der spezifischen Immunantwort. Pathologische Veränderungen dieser Abgrenzung können zu schwerwiegenden Autoimmunerkrankungen wie beispielsweise Diabetes Mellitus, Rheumatischer Arthritis oder Multipler Sklerose führen. Um unerwünschte (Auto-) Immunreaktionen zu verhindern, existieren verschiedene Formen von peripheren Toleranzmechanismen, die durch viele Transkriptionsfaktoren wie z. B. ICER (inducible cAMP early repressor), NFAT (nuclear factor of activated T cells) und Foxp3 (forkhead box protein p3) kontrolliert werden. Foxp3+ regulatorische T-Zellen (Tregs) sind spezialisierte immun-suppressive Lymphozyten, welche die Aktivierung anderer Immunzellen unterdrücken können. Einer der möglichen Mechanismen ist der Transfer zyklischen Adenosin-Monophosphats (cAMP) von Tregs in konventionelle T- und B-Lymphozyten. Die erhöhte intrazelluläre Konzentration an cAMP führt in Effektorzellen zur Induktion und Kerntranslokation von ICER. Der transkriptionelle Repressor ICER supprimiert die Expression vieler NFAT-regulierter Gene und hemmt darüber hinaus die Induktion der NFATc1/αA-Isoform selbst. Diese Isoform wird speziell in pro-inflammatorischen Effektorzellen hochreguliert und ist maßgeblich an deren spezifischem transkriptionellen Programm beteiligt. Foxp3 ist ein zentraler Faktor für die Bildung und Funktion sowohl Thymus-generierter nTregs als auch peripher (TGFβ-) induzierter iTregs. Die Kontrolle des Foxp3-Gens wird in iTregs – überraschenderweise aber nicht in nTregs – durch NFAT-Faktoren reguliert. Allerdings hemmt Foxp3 durch eine negative Rückkopplung wiederum die Induktion und Aktivität von NFATc1/αA. Dies stellt somit ein weiteres Regulativ dar, wobei Foxp3 nicht nur die Plastizität, sondern auch die Funktion von immun-suppressiven T-Zellen steuert. Zusätzlich regulieren die verschiedenen NFAT-Faktoren auch die Antigen präsentierenden dendritischen Zellen (DCs). Während NFATc1 und NFATc2 die Differenzierung und Proliferation von DCs beeinflussen, reguliert NFATc3 deren Zytokinexpression und steuert indirekt auch die nachfolgende T-Zell-Immunantwort. Die Kontrolle der Genregulation in Immunzellen durch die Transkriptionsfaktoren ICER, NFAT und Foxp3 erfüllt somit spezifische Funktionen der Immunität, reguliert aber gleichzeitig wichtige Aspekte der peripheren Toleranz, um schädliche (Auto-) Immunreaktionen zu verhindern.
Background
Investigation of the expression of an intestinal stem cell marker in esophageal adenocarcinomas (EAC) with and without Barrett's Esophagus (BE), with respect to a cancer stem cell (CSC) hypothesis.
Materials and methods
Expression of a putative intestinal stem cell marker LgR5 was analyzed in esophageal cancer specimen (n = 70: 41 EAC with BE, 19 EAC without BE, and n = 10 esophageal squamous-cell carcinomas, ESCC) and in the adenocarcinoma cell line OE-33. Ki-67 and Cdx-2 were co-labelled with LgR5 in double staining experiments. Immunhistochemical expression results were confirmed by RT-PCR and correlated with tumor stage and five-year survival rates.
Results
LgR5was found expressed in 35 of 41 (85%) EAC with BE and in 16 of 19 (81%) EAC without BE. By contrast, LgR5 was not found to be expressed in ESCC. Quantification of immunolabeling showed 15% LgR5+ cells in EAC with BE, 32% LgR5+ cells in adjacent BE and 13% in EAC without BE. Immunofluorescence double staining experiments with LgR5 and Ki-67 revealed a subpopulation (~5%) of proliferating LgR+/Ki-67+ cells. On mRNA-level, expression of LgR5 was higher in BE in comparison to EAC (p = 0.0159). High levels of LgR5 expression in BE associated EAC were associated with poorer survival in univariate analysis.
Conclusion
The stem cell marker LgR5 is expressed in EAC, irrespective of association with BE, and appears to have negative impact on survival. The subset of proliferating LgR5+ cells (<5%) might resemble rapidly cycling CSCs, which needs to be substantiated in further investigations.
Background
Renal cell carcinoma (RCC) is marked by high mortality rate. To date, no robust risk stratification by clinical or molecular prognosticators of cancer-specific survival (CSS) has been established for early stages. Transcriptional profiling of small non-coding RNA gene products (miRNAs) seems promising for prognostic stratification. The expression of miR-21 and miR-126 was analysed in a large cohort of RCC patients; a combined risk score (CRS)-model was constructed based on expression levels of both miRNAs.
Methods
Expression of miR-21 and miR-126 was evaluated by qRT-PCR in tumour and adjacent non-neoplastic tissue in n = 139 clear cell RCC patients. Relation of miR-21 and miR-126 expression with various clinical parameters was assessed. Parameters were analysed by uni- and multivariate COX regression. A factor derived from the z-score resulting from the COX model was determined for both miRs separately and a combined risk score (CRS) was calculated multiplying the relative expression of miR-21 and miR-126 by this factor. The best fitting COX model was selected by relative goodness-of-fit with the Akaike information criterion (AIC).
Results
RCC with and without miR-21 up- and miR-126 downregulation differed significantly in synchronous metastatic status and CSS. Upregulation of miR-21 and downregulation of miR-126 were independently prognostic. A combined risk score (CRS) based on the expression of both miRs showed high sensitivity and specificity in predicting CSS and prediction was independent from any other clinico-pathological parameter. Association of CRS with CSS was successfully validated in a testing cohort containing patients with high and low risk for progressive disease.
Conclusions
A combined expression level of miR-21 and miR-126 accurately predicted CSS in two independent RCC cohorts and seems feasible for clinical application in assessing prognosis.
Clear cell renal cell carcinoma (ccRCC) characterized by a tumor thrombus (TT) extending into the inferior vena cava (IVC) generally indicates poor prognosis. Nevertheless, the risk for tumor recurrence after nephrectomy and thrombectomy varies. An applicable and accurate prediction system to select ccRCC patients with TT of the IVC (ccRCC/TT) at high risk after nephrectomy is urgently needed, but has not been established up to now. To our knowledge, a possible role of microRNAs (miRs) for the development of ccRCC/TT or their impact as prognostic markers in ccRCC/TT has not been explored yet. Therefore, we analyzed the expression of the previously described onco-miRs miR-200c, miR-210, miR-126, miR-221, let-7b, miR-21, miR-143 and miR-141 in a study collective of 74 ccRCC patients. Using the expression profiles of these eight miRs we developed classification systems that accurately differentiate ccRCC from non-cancerous renal tissue and ccRCC/TT from tumors without TT. In the subgroup of 37 ccRCC/TT cases we found that miR-21, miR-126, and miR-221 predicted cancer related death (CRD) accurately and independently from other clinico-pathological features. Furthermore, a combined risk score based on the expression of miR-21, miR-126 and miR-221 was developed and showed high sensitivity and specificity to predict cancer specific survival (CSS) in ccRCC/TT. Using the combined risk score we were able to classify ccRCC/TT patients correctly into high and low risk cases. The risk stratification by the combined risk score (CRS) will benefit from further cohort validation and might have potential for clinical application as a molecular prediction system to identify high- risk ccRCC/TT patients.
Adamantiades-Behçet disease (ABD) is a chronic, multisystemic, recurrent, inflammatory vascular disorder of unknown etiology. Patients with symptoms initially appearing at the age of 16 or less are considered as cases of juvenile-onset ABD (JABD). JABD is relatively rare compared to ABD of adults, and only case reports and case studies have been published regarding this subtype of the disease. Epidemiology, clinical features, diagnosis and treatment of JABD are discussed in this review.
Contribution of adventitia-derived stem and progenitor cells to new vessel formation in tumors

Blocking tumor vascularization has not yet come to fruition to the extent it was hoped for, as angiogenesis inhibitors have shown only partial success in the clinic. We hypothesized that under- appreciated vascular wall-resident stem and progenitor cells (VW-SPCs) might be involved in tumor vascularization and influence effectiveness of anti-angiogenic therapy. Indeed, in patient samples, we observed that vascular adventitia-resident CD34\(^+\) VW-SPCs are recruited to tumors in situ from co-opted vessels. To elucidate this in detail, we established an ex vivo model using concomitant embedding of multi-cellular tumor spheroids (MCTS) and mouse aortic rings (ARs) into collagen
gels, similar to the so-called aortic ring assay (ARA). Moreover, ARA was modified by removing the ARs’ adventitia that harbors VW-SPCs. Thus, this model enabled distinguishing the contribution of VW-SPCs from that of mature endothelial cells (ECs) to new vessel formation. Our results show that the formation of capillary-like sprouts is considerably delayed, and their number and network formation were significantly reduced by removing the adventitia. Substituting iPSC-derived neural spheroids for MCTS resulted in distinct sprouting patterns that were also strongly influenced by the
presence or absence of VW-SPCs, also underlying the involvement of these cells in non-pathological vascularization. Our data suggest that more comprehensive approaches are needed in order to block all of the mechanisms contributing to tumor vascularization.
NFAT transcription factors play critical roles in gene transcription during immune responses. Besides regulation of lymphokine promoters in T lymphocytes, NFAT factors are also expressed in other cell types and regulate the activity of numerous genes that control the generation of cardiac septa and valves in embryonic heart, the formation of blood vessels, the outgrowth of neuronal axons and the differentiation of osteoclasts during bone formation [10, 24]. Here we show that the induction of NFATc/αA in effector T cells is controlled by a strong inducible promoter, P1. It results in splicing of exon 1 to exon 3 transcripts and, in concert with the activity of a poly A site downstream of exon 9, leads to the massive synthesis of NFATc/αA in effector Th1 cells. A second, weak promoter, P2, lies in front of exon 2 and directs the synthesis of longer NFAT β isoforms. Both P1 and P2 direct the synthesis of three different RNAs: αA, αB, αC and βA, βB, βC correspondingly. The B and C isoforms arise from alternative splicing and poly A addition at the distal site pA2. P1 but not P2 activity is autoregulated by NFAT factors which bind to two tandemly arranged NFAT sites within P1 and enhance its induction. In resting T cells, the NFATc1/β RNAs are the most prominent nfatc1 transcripts and their synthesis is reduced upon T-cell activation. However, following activation in primary effector T cells or in T-cell lines of human or murine origin, a 15–20-fold induction of NFATc1/αA RNA was detected, whereas only a 2–5-fold increase was observed for the NFATc1/αB or NFATc1/αC RNAs. Optimal induction of P1 promoter require involving of a persistent increase in free cytosolic Ca2+ induced by ionomycin, which stimulates the nuclear translocation and transcriptional activation of all NFATc factors and phorbol esters, which activate protein kinase C and other protein kinase pathways in T cells. This suggests that both TCR and co-receptor signals contribute to give full P1 nfatc1 induction. Because NFATc1/αA induction is unaffected in NFATc2+c3 double-deficient T cells, NFATc1 autoregulates its own synthesis by controlling P1 activity and NFATc1/αA induction. P1 promoter contains tandemly arranged NFAT core binding motif TGGAAA to witch bind monomeric NFATc1 proteins and numerous conservative binding sites of other transcriptional factors like CREB, Fos, ATF-2, Sp1, NF-kB and GATA suggesting complex multi-factor regulation of NFATc1 gene. We also highlight that initial phase of nfatc1 transcription in naive CD4+ T cells is controlled by the promoter P2 which is constitutively active in resting T cells. The activation of resting T cells results in a decrease of P2 and the induction of P1 activity and, under optimal conditions, in the predominant synthesis of NFATc1/αA in effector T cells. In addition to the high concentrations of poly A factors required for optimal pA1 function, the levels of transcription factors, in particular NFATs, must also increase for P1 induction. That could be explained by achievement of certain threshold levels for transcriptional activation. Finally, the altered transactivation potential of NFATc1/αA suggests a specific role for this NFATc1 protein in gene control, such as in Th1 effector cells where NFATc1/αA is synthesized at high concentrations.
In der vorliegenden Arbeit wurden die Expressionshäufigkeiten von MAGE-A-Antigenen, E -Cadherin, Laminin-5-gamma-2, MMP2 und MMP9 in Plattenepithelkarzinomen im Kopf- und Halsbereich untersuchen. Hierbei findet der Vergleich zwischen Primärtumoren und korrespondierenden Lymphknotenmetastasen besondere Beachtung. Um die Hypothese zu verifizieren, dass die o.g. Parameter einen signifikanten Einfluss auf die Progression und Metastasierung haben, wird der Zusammenhang zwischen den in der vorliegenden Arbeit gewonnenen Ergebnissen und diversen klinischen Parametern mittels Korrelationsanalyse untersucht.
Four molecules of the tumor suppressor p53 assemble to cooperatively bind proapoptotic target genes. The structural basis for cooperativity consists of interactions between adjacent DNA binding domains. Mutations at the interaction interface that compromise cooperativity were identified in cancer patients, suggesting a requirement of cooperativity for tumor suppression. We report on an analysis of cooperativity mutant p53(E177R) mice. Apoptotic functions of p53 triggered by DNA damage and oncogenes were abolished in these mice, whereas functions in cell-cycle control, senescence, metabolism, and antioxidant defense were retained and were sufficient to suppress development of spontaneous T cell lymphoma. Cooperativity mutant mice are nevertheless highly cancer prone and susceptible to different oncogene-induced tumors. Our data underscore the relevance of DNA binding cooperativity for p53-dependent apoptosis and tumor suppression and highlight cooperativity mutations as a class of p53 mutations that result in a selective loss of apoptotic functions due to an altered quaternary structure of the p53 tetramer.
Im Rahmen der vorliegenden Arbeit wurde eine detaillierte Analyse des Chromosom 17p-Status in DLBCL durchgeführt, welches das TSG TP53 beinhaltet. Eine monoallelische Deletion dieses Gens fand sich in 16% der Patienten (28/172), darüber hinaus lag in 3% (6/172) der Patienten ein Verlust in Chromosom 17p ohne Alteration des TP53-Lokus vor. Zur Untersuchung des zweiten TP53-Allels wurden an allen 28 Patienten mit sowie an 27 Patienten ohne TP53-Deletion Mutationsanalysen durchgeführt. Eine vollständige Inaktivierung des TP53-Gens durch Deletion und Mutation konnte in 46% der 17p-deletierten Fälle (13/28) gezeigt werden. In 54% der 17p-deletierten DLBCL (15/28) lag lediglich eine monoallelische TP53-Deletion ohne gleichzeitige Mutation des zweiten Allels vor. 26% der DLBCL mit nicht-deletiertem TP53 (7/27) zeigten eine monoallelische Mutation des TP53-Gens. Diese Ergebnisse deuteten auf weitere TSG in der chromosomalen Region 17p13 hin. Mit der Durchführung einer detaillierten Deletionsanalyse von Chromosom 17p13.1 bis 17p13.3 wurden folgende Ergebnisse erzielt. In insgesamt 41% der DLBCL mit nicht-deletiertem TP53-Gen (11/27) konnte eine minimal deletierte Region identifiziert werden, welche die chromosomale Bande 17p13.3 einschließlich des genetischen Lokus des TSG HIC1 betraf. Eine Deletion von TP53 war in allen untersuchten Fällen mit einem subtotalen Verlust des kurzen Armes von 17p einschließlich der o.g. minimal deletierten Region verbunden. Mehrere Hinweise deuten darauf hin, dass die Inaktivierung von HIC1 eine entscheidende Rolle in der Pathogenese von DLBCL einnimmt. In 55% der untersuchten DLBCL (30/55) wurde eine Hypermethylierung des HIC1-Promoters nachgewiesen. In 90% der HIC1-hypermethylierten DLBCL (27/30) lag zudem eine gleichzeitige Deletion des HIC1-Lokus vor, was eine biallelische Inaktivierung des TSG, analog der „two-hit“-Hypothese nach Knudson, nahe legte. Eine vollständige durch Hypermethylierung und Deletion verursachte HIC1-Inaktivierung konnte auch in sieben Fällen mit Wildtyp TP53-Status gezeigt werden. Die Überlebenszeit von Patienten mit einer gleichzeitigen HIC1- und TP53-Inaktivierung war im Vergleich zu Patienten mit alleiniger Inaktivierung von TP53 deutlich verkürzt. Es konnte somit gezeigt werden, dass in den untersuchten DLBCL, über die klinisch bedeutsame und prognostisch relevante Inaktivierung von TP53 hinaus, ein weiteres TSG unabhängig von oder in Kombination mit TP53 alteriert ist, und einen Einfluss auf die Überlebenszeit der Patienten hat.
Follikuläre Lymphome (FL) zählen zu den Non-Hodgkin-Lymphomen und stellen die
größte Untergruppe der B-Zell-Lymphome dar. Bedingt durch ihren meist indolenten
Verlauf werden sie oft erst in einem fortgeschrittenen klinischen Stadium III/IV
diagnostiziert und stellen dann eine systemische Erkrankung dar.
Gelegentlich wird in der histopathologischen Untersuchung eines befallenen
Lymphknotens eine nur partielle Infiltration beobachtet, die häufig auch in den
angeschlossenen Stagingmaßnahmen mit einer nur lokalen Tumorausbreitung (klinisches
Stadium I/II) assoziiert ist. Ein solches lokal begrenztes Stadium kann gemäß der
Standard-Behandlungsprotokolle mit einer alleinigen Strahlentherapie ausreichend
kontrolliert werden.
Ziel der vorliegenden Arbeit war es zum einen, eine mögliche Assoziation einer nur
partiellen Lymphknoteninfiltration beim FL mit einem lokal begrenzten klinischen Stadium
zu untersuchen. Zum anderen sollte die Inzidenz einer nur partiellen Lymphknoten-
Infiltration beim FL bestimmt werden.
Der Vergleich der Studienkohorte mit einer nur partiellen Lymphknoteninfiltration, definiert
als zumindest ein vollständig erhaltener Lymphfollikel, mit der Kontrollkohorte zeigte
einen hochsignifikanten Unterschied: In der Studienkohorte befanden sich 38 von 40 Fälle
(95%) in einem lokalen Stadium, wohingegen die Kontrollkohorte mit vollständiger
Lymphknoteninfiltration nur bei 10 von 49 Patienten (20%) ein lokales Krankheitsstadium
(p<0.001) aufwies.
Um die erhaltenen Ergebnisse zu validieren, wurden alle FL Grad 1-3A aus dem
exemplarischen Jahr 2001 untersucht. Hier zeigte sich in 34 Fällen (11 %) eine nur
partielle Infiltration. In allen 18 Fällen mit mindestens einem vollständig erhaltenen
reaktiven Keimzentrum lag in Übereinstimmung mit der initialen Studienkohorte ein lokales
Krankheitsstadium I/II vor (p<0.001).
Die erhaltenen Ergebnisse zeigen eindrücklich, dass follikuläre Lymphome mit einem nur
partiellen Befall der Lymphknoten häufig mit einem (noch) lokalen klinischen Stadium
assoziiert sind. In diesen Fällen käme eine alleinige Bestrahlung als Therapieoption in
Betracht.
In der vorliegenden Arbeit wurden einerseits zelltypspezifische Untersuchungen der mitochondrialen DNA zur Bestimmung der Deletionslast, als Marker für oxidativen Stress, andererseits neuroinflammations-assoziierte Genexpressions-Analysen am humanen post mortem Hirngewebe von Patienten mit unterschiedlichen Stadien der Alzheimer Erkrankung durchgeführt. Als Grundlage hierzu diente das noch nicht gänzlich aufgeschlüsselte Konzept der selektiven Vulnerabilität unterschiedlicher Hirnregionen. Dabei zeigte sich, dass der Hippocampus, eine auf lichtmikroskopischer Ebene sehr früh befallene Region, auch molekularbiologisch deutliche Unterschiede gegenüber resistenten Regionen wie z.B. dem Kleinhirn aufweist.
Die Rolle der DNA-Methylierung in der Kontrolle des MHC-II-spezifischen Typ-IV-Promotors in Thymomen
(2004)
Die Moleküle des „major histocompatibility complex“ (MHC) spielen in der thymischen T-Zell-Reifung eine zentrale Rolle. Durch Interaktion des T-Zell-Rezeptors auf der unreifen T-Zelle und den Co-Rezeptoren CD4 und CD8 mit dem MHC auf dem Thymusepithel werden die zwei wichtigsten Reifungs- und Selektionsprozesse, positive und negative Selektion, vermittelt. Störungen dieser Selektionsmechanismen können zu Immundefekten oder Autoimmunphänomenen führen. Die Regulierung der MHC IIExpression erfolgt hauptsächlich transkriptionell und wird vor allem durch den sogenannten Class II Transaktivator (CIITA) bestimmt. Die Transkription von CIITA wird über vier alternative, gewebsspezifische Promotoren durch alternatives splicing reguliert. Unter diesen Splicevarianten ist der CIITA Typ IV Promotor für die konstitutive MHC II-Expression in Thymusepithelzellen und für die IFN-g induzierte Expression in anderen Geweben verantwortlich. Thymome sind menschliche Tumoren des Thymusepithels, welche häufig mit einer paraneoplastischen Myasthenia gravis (MG) assoziiert sind. Die paraneoplastische MG zeigt eine starke positive Korrelation mit der Fähigkeit eines Thymoms, reife CD4+ T-Zellen zu produzieren und in das periphere Blut zu exportieren. Thymome weisen aus bislang ungeklärten Gründen häufig eine starke Reduktion der epithelialen MHC II- Expression auf. Ziel dieser Arbeit war die Beantwortung der Frage, ob eine DNA-Methylierung der Promotorregion des MHC-Klasse II Transaktivators (CIITA) Typ IV daran beteiligt ist. Hierzu wurden 27 Thymome mit sowohl hoher und als auch niedriger MHC II-Expression und 9 Normalthymi aus unterschiedlichen Altersgruppen untersucht. Die Ergebnisse zeigten, dass der CIITA Typ IV Promotor in kindlichen Kontrollthymi vollständig unmethyliert ist, während die Thymi erwachsener Kontrollpersonen eine vermehrte Methylierung aufweisen. Im Gegensatz dazu war der CIITA Typ IV in nahezu allen Thymomen stark hypermethyliert. Da aber kein signifikanter Unterschied in der Methylierung zwischen Thymomen mit hoher und solchen mit niedriger MHC II-Expression festgestellt werden konnte, lassen diese Ergebnisse vermuten, dass eine Hypermethylierung des CIITA Typ IV Promotors sehr wahrscheinlich nicht der alleinige Grund für die verminderte MHC II-Expression in Thymomen ist, sondern das auch andere Regulationsmechanismen wie zum Beispiel Histon-Acetylierung beteiligt sein könnten beteiligt sein könnten.
Die Entdeckung der Autoimmunkrankheit APECED führte zur Entdeckung des verantwortlichen Gens AIRE (autoimmune regulator), das einen Einblick in die Verhinderung oder Entstehung von Autoreaktivität im Thymus bietet. Durch Aktivierung des AIRE-Gens und Expression des AIRE-Proteins werden organspezifische Selbstantigene in medullären Thymusepithelzellen exprimiert und entweder direkt oder nach Transfer auf dendritsche Zellen unreifen T-Zellen präsentiert. Durch Elimination autoreaktiver Zellen entsteht Toleranz gegenüber den körpereigenen Geweben. Da das Immunsystem bei steigendem Alter tiefgreifende Veränderungen erfährt, die zum Teil durch die Thymusinvolution bedingt sind, ist es von Interesse, den Verlauf von AIRE über verschiedene Altersstufen hinweg zu untersuchen. Zielsetzungen dieser Arbeit waren daher die quantitative und räumliche Untersuchung der AIRE+ -Zellen im Alter und im Kontext einer prototypischen organspezifischen Autoimmunerkrankung, der Myasthenia gravis. Diese Fragestellungen wurden bei Normalthymi, entzündlich veränderten Thymi mit assoziierter Myasthenie und an B1-Thymomen untersucht. Als Ergebnis konnte erarbeitet werden, dass der Gehalt an AIRE+ -Zellen mit dem Alter abnimmt, wobei bis in die neunte Lebensdekade noch AIRE+ -Zellen vorhanden waren. Der Gehalt an AIRE+ Zellen stellte sich als stärker mit dem Lebensalter korreliert dar als beispielsweise die Thymusfläche. AIRE scheint damit ein mit der Thymusinvolution eng assoziiertes Gen zu sein. Es fand sich eine hochsignifikante Co-Lokalisation von AIRE+ Zellen und Hassall-Körperchen, die besonders durch die Untersuchungen von Typ B1 Thymomen mit und ohne Hassall-Körperchen bestätigt werden konnte. In Übereinstimmung mit aktuellen Forschungsergebnissen in Mausmodellen fand sich kein Zusammenhang zwischen der Anzahl oder der räumlichen Anordnung regulatorischer T-Zellen und AIRE+ -Zellen. Myoidzellen nahmen mit zunehmendem Alter ebenfalls ab. Ihre Rolle in der Entstehung der Myasthenie ist bei Patienten mit early und late onset wahrscheinlich unterschiedlich; die abnehmende Zahl an Myoidzellen stellt möglicherweise vor allem bei Patienten mit late onset einen Risikofaktor dar.
„Black esophagus“ oder „akute Ösophagusnekrose“ (AÖN) ist eine seltene Erkrankung, die sich makroskopisch durch eine zirkumferente Schwarzverfärbung der Ösophagusmukosa mit abruptem Ende am gastroösophagealen Übergang auszeichnet. Die genaue Pathogenese ist unbekannt; es werden multifaktorielle Einflüsse wie z. B. Säurereflux, Ischämie und verringerte Schutzmechanismen der Mukosa als mögliche Ursachen diskutiert.
Vorgestellt werden 2 Obduktionsfälle, die typische Befunde einer AÖN aufwiesen. Zusätzlich hatten Fall 1 eine Candida-Infektion und Fall 2 eine Appendizitis, sodass eine infektiöse Genese in beiden Fällen eine Rolle gespielt haben könnte.
In der vorliegenden Arbeit wurden der klinische Verlauf der Tumorerkrankung von 40 Kindern, die an einem Ependymom erkrankten, erfasst und 58 Tumorproben mittels immunhistochemischer Methoden untersucht. Im Mittelpunkt dieser Untersuchungen standen die Überprüfung der p53-Akkumulation, der Proliferationsaktivität mit Hilfe des Antikörpers MIB-1 sowie der Expression von HLA-DR durch Tumorzellen und eine Einschätzung der prognostischen Aussagekraft dieser Faktoren. Weiterhin wurde die EMA-Expression untersucht und eine Schwerpunkt auf die Analyse von Verteilung und Proliferationsaktivität der Zellen des Mikroglia(MG)/Makrophagen-Systems in kindlichen Ependymomen gelegt. Das mittlere Alter bei Diagnosestellung lag bei 61 Monaten (8 Monate bis 15 4/12 Jahre, Median: 41 Monate), eine Geschlechtspräferenz lag nicht vor. 22,5% der Primärtumoren waren supratentoriell, 60% infratentoriell und 17,5% spinal lokalisiert. 52,5% der Primärtumoren waren anaplastische Ependymome (WHO Grad III), 40% Ependymome (WHO Grad II), 7,5% myxopapilläre Ependymome (WHO Grad I). Eine totale Resektion konnte in 30,8% bei Primärtumoren und in 27,3% bei Rezidivtumoren erzielt werden. Das mittlere Nachbeochtungsintervall betrug 52 Monate (6-163 Monate), das mittlere progrssionsfreie Interval (PFS) 28 Monate und der mittlere Überlebenszeitraum (OS) 64 Monate. Tumorrezidive entwickelten sich mit einer Ausnahme eines Spätrezidives nach 84 Monaten innerhalb von 36 Monaten nach der operativen Tumorentfernung. Von den klinischen Variablen weist einzig das Resektionsausmaß eine prognostische Bedeutung auf (total vs. inkomplett: p=0.035 bezüglich des mittleren PFS, p=0.12 bezüglich des mittleren OS). Alle anderen klinischen Variablen, einschließlich des Tumorgrades nach WHO, zeigen keinen Einfluss auf den Verlauf der Tumorerkrankung. p53-Akkumulation kommt in 56,9% der untersuchten Tumorproben vor und korreliert mit dem Tumorgrad nach WHO, dem MIB-1 Labeling Index (LI) und der Lokalisation. Der MIB-1 LI zeigt eine große Streubreite (1,4% - 63,3%) und weist entsprechend des untersuchten Patientenkollektivs im Gegensatz zu Ependymomen des Erwachsenenalters einen hohen Mittelwert und Median auf (21,8% bzw. 19,8%). Der MIB-1 LI korreliert statistisch signifikant mit dem WHO-Graduierungssystem, der Lokalisation und wie bereits erwähnt mit der p53-Akkumulation. Unter den immunhistochemisch untersuchten Variablen kommt lediglich dem MIB-1 LI im untersuchten Patientenkollektiv eine gewisse prognostische Aussagekraft zu. Statistische Signifikanz wird allerdings nicht erreicht (MIB-1 LI <10% vs. MIB-1 LI >10%: p=0.17 bezüglich des mittleres PFS). Erstmals wurde in 80,4% der untersuchten ependymalen Tumoren auch eine Proliferation von Zellen des MG/Makrophagen-Systems nachgewiesen. Bei weitgehend konstant niedriger absoluter Anzahl proliferierender Zellen des MG/Makrophagen-Systems existiert kein Unterschied diesbezüglich zwischen den Tumorgraden wie kürzlich bei astrozytären Tumoren nachgewiesen. Die Verteilung der Zellen des MG/Makrophagen-Systems zeigte bei sehr heterogenem Verteilungsmuster häufig eine septale Anordnung insbesondere der amöboiden MG und der Makrophagen. Es existieren unterschiedliche Verteilungsmuster mikroglialer Zellen und Makrophagen, die sich aber anderen Parametern nicht zuordnen lassen mit der Ausnahme der MG/Makrophagen-Zelldichte in Primärtumoren. Obwohl eine HLA-DR Expression durch Tumorzellen auch in anderen Neoplasien des ZNS häufig vorkommt, wurden Ependymome diesbezüglich bisher nicht untersucht. In 52,6% aller Tumorproben finden sich in der vorliegenden Arbeit HLA-DR exprimierende Tumorzellen. Eine EMA-Expression zeigt sich in 84,5% der untersuchten Fälle und ist damit ein Charakteristikum ependymaler Neoplasien, das auch als differentialdiagnostisches Kriterium angesehen werden kann. Die Ergebnisse der vorliegenden Arbeit bekräftigen den hohen prognostischen Stellenwert einer totalen Resektion in der Therapie ependymaler Tumoren des Kindesalters. Weiterhin wird die Bedeutung der Proliferationsaktivität für den klinischen Verlauf der Tumorerkrankung unterstrichen. Von großem wissenschaftlichem Interesse ist weiterhin die Klärung der Ursache und Bedeutung der häufig anzutreffenden p53-Akkumulation sowie der immunologischen Bedeutung der Zellen des MG/Makrophagen-Systems im Rahmen der Tumorabwehr.
Purpose
Patients suffering from aggressive systemic peripheral lymphoma with primary central nervous system involvement (PCL) are a rare and sparsely investigated population. Recommended treatment regimens include a combination of intrathecal and systemic chemotherapy as well as whole brain radiotherapy while offering relatively poor survival.
Methods
We conducted a single-center retrospective study that analyzed safety and outcome of 4 + 4 cycles Rituximab (R)-CHOP and R-high-dose Methotrexate (HD-MTX) for newly diagnosed, transplant-eligible patients ("Ping-Pong"), followed by Cytarabine (AraC)/Thiotepa (TT), BCNU/TT, and autologous hematologic stem cell transplantation (aHSCT). We retrospectively analyzed a set of 16 patients with high-intermediate or high-risk IPI status.
Results
Overall response rate to Ping-Pong was 100% measured by CT/MRI, including 93.75% complete remissions after BCNU/TT followed by PBSCT. One patient failed to qualify for high-dose chemotherapy due to progression when receiving Cytarabine/TT. All patients experienced grade III adverse events, 3 of them a grade IV adverse event. Estimated progression-free survival is 93.75% after a 4.8-year follow-up currently.
Conclusion
Our study suggests high effectivity of R-CHOP with mid-cycle MTX with aHSCT consolidation towards acceptable OS results in this challenging patient population.
Marginalzonen-B-Zell-Lymphome vom MALT-Typ stellen die häufigste Form primär extranodaler Non-Hodgkin-Lymphome dar. Sie entstehen auf dem Boden einer chronischen infektiösen oder autoimmunen Entzündung in hierdurch sekundär erworbenem lymphatischen Gewebe. Eine charakteristische und die häufigste bei diesen Tumoren beobachtete Translokation ist die t(11;18)(q21;q21). Hierbei fusioniert API 2, ein Apoptoseinhibitor-Gen auf 11q21 mit MALT 1 auf 18q21, einer humanen Paracaspase. Mit dem Ziel, letztlich die Funktion und Rolle des durch die Translokation trunkierten MALT 1-Gens untersuchen zu können, wurde bei dieser Arbeit ein Vektorsystem konstruiert, mit dem das MALT 1-Bruchstück stabil und nachweislich in humane B-Zelllinien transfiziert werden kann. An und mit diesen Transfektanten können nun vielfältige Analysen durchgeführt werden, um weitere Einsichten in die Lymphompathogenese zu gewinnen. Im Rahmen dieser Arbeit wurden noch Untersuchungen zum Wachstumsverhalten und Zellzyklus von das trunkierte MALT 1-Gen überexprimierenden Zellen durchgeführt. Es zeigte sich, dass diese Zellen im Vergleich zu das intakte Gen exprimierenden Zellen stärker und konstanter proliferierten. Dies legte den Schluss nahe, dass in der Genstruktur vor dem Bruchpunkt proliferationshemmende und regulatorische Domänen liegen könnten, welche schließlich durch die Translokation eliminiert werden. Hierdurch könnte der verbleibende Anteil des Gens unreguliert zur Tumorgenese beitragen.
Zur Identifizierung geeigneter Routinemarker für die Prognose von Ependymompatienten führten wir immunhistochemische Untersuchungen und statistische Auswertungen an Ependymomen und Daten von 32 Erwachsenen und 23 pädiatrischen Patienten durch. Davon wurden bei drei Tumoren auch Rezidive untersucht, so dass insgesamt 59 Ependymome in die Untersuchung eingeschlossen wurden. Im Einzelnen handelte es sich um 11 myxopapilläre Ependymome, 6 Subependymome, 19 Ependymome und 23 anaplastische Ependymome. Die größten Fallgruppen bildeten pädiatrische Patienten unter drei Jahren und Erwachsene zwischen 50 und 70 Jahren. Bei Kindern war mit 45,8% die infratentorielle, bei Erwachsenen mit 65% die spinale Tumorlokalisation am häufigsten. Die untersuchten spinalen Ependymome entsprachen zu gleichen Teilen myxopapillären Ependymomen WHO Grad I und Ependymomen WHO Grad II. In supratentorieller Lage fanden sich mit 67% überwiegend anaplastische Ependymome WHO Grad III. Auch bei den infratentoriell gelegenen Ependymomen waren mit 63% die Mehrzahl anaplastische Ependymome, daneben fanden sich 29,6% Ependymome WHO Grad II. Beim Vergleich des von uns definierten und bestimmten Ki67-Scores als Zeichen für die Ependymomproliferation und der immunhistochemischen Positivität für HCK fiel nach Anwendung des Chi-Quadrat-Tests mit p=0,067 ein deutlicher Trend zu schwächerer punktförmiger Positivität bei höherem Ki67-Score auf. Dieser Trend setzte sich in der Erwachsenengruppe separat fort, während er in der Kindergruppe allein nicht nachweisbar war. In der Erwachsenengruppe war mit 28% ein deutlicher Anteil myxopapillärer Ependymome vorhanden, welche bei den Kindern nur 8% ausmachten.Möglicherweise spielt die veränderte HCK-Expression in der Subgruppe der myxopapillären Ependymome eine Rolle. Unsere Untersuchungen zeigten außerdem mit p=0,057 einen deutlichen Trend zu längerem Überleben bei immunohistochemischer DBC1-Negativität. Die Multivarianzanalyse mittels Cox-Regression wies eine Positivität für DBC1 als unabhängigen Risikofaktor für eine kürzere Überlebenszeit nach. Des Weiteren konnte eine mit p=0,013 signifikante Korrelation zwischen immunhistochemischer Positivität für DBC1 und höherem Ki67-Score gezeigt werden. Auch mit höherem WHO-Grad korrelierte die DBC1-Positivität mit p=0,009. Besonders infratentoriell gelegene Ependymome zeigten DBC1-Reaktivität. Hier treten bekannterweise häufiger anaplastische Ependymome mit höherem Proliferationsindex auf. Unsere Ergebnisse legen somit die Eignung des Markers DBC1 als immunhistochemische Routineuntersuchung für die Beurteilung der vom Resektionsstatus unabhängigen Prognose und Überlebenszeit von Ependymompatienten nahe.
Due to the wide variety of benign and malignant salivary gland tumors, classification and malignant behavior determination based on histomorphological criteria can be difficult and sometimes impossible. Spectroscopical procedures can acquire molecular biological information without destroying the tissue within the measurement processes. Since several tissue preparation procedures exist, our study investigated the impact of these preparations on the chemical composition of healthy and tumorous salivary gland tissue by Fourier-transform infrared (FTIR) microspectroscopy. Sequential tissue cross-sections were prepared from native, formalin-fixed and formalin-fixed paraffin-embedded (FFPE) tissue and analyzed. The FFPE cross-sections were dewaxed and remeasured. By using principal component analysis (PCA) combined with a discriminant analysis (DA), robust models for the distinction of sample preparations were built individually for each parotid tissue type. As a result, the PCA-DA model evaluation showed a high similarity between native and formalin-fixed tissues based on their chemical composition. Thus, formalin-fixed tissues are highly representative of the native samples and facilitate a transfer from scientific laboratory analysis into the clinical routine due to their robust nature. Furthermore, the dewaxing of the cross-sections entails the loss of molecular information. Our study successfully demonstrated how FTIR microspectroscopy can be used as a powerful tool within existing clinical workflows.
SUMMARY GABP is a heterodymeric member of Ets-family transcription factors. It consists of two subunits – GABPa which contains DNA binding domain and GABPb, which provides transcriptional activation domain and nuclear localization signal. GABPa/b complex is essential for transcriptional activation of multiple lineage-restricted and housekeeping genes, several viral genes, and in some cases might function as transcriptional repressor. Large variety of data indicates involvement of GABP in the complex regulation of cell growth, specified by quiescence, stimulation/proliferation, apoptosis and senescence. Expression level of GABPa subunit is rapidly increased when resting cells enter S-phase, and GABPa/b complex is critical to promote the continuity of the cell cycle. Conditional inactivation of GABPa expression in mouse embryonic fibroblasts results in a complete block of proliferation and acquisition of senescence-like phenotype. However, the influence of GABP on the other cell growth determinant – the apoptosis – remains largely obscure. Therefore we aimed to investigate the influence of GABPa/b expression level on the cell growth in vitro. Using siRNA approach we achieved efficient but only transient down-regulation of GABPa expression which precluded further cell growth studies. Persistent increase of the expression of GABPb subunit only resulted in a positive effect on the cell growth speed. Simultaneous conditional overexpression of both GABPa and GABPb subunits though, strongly reduced the growth of the affected cell cultures in reversible and in expression level dependent manner. Interestingly, GABPa/b overexpressing cells did show neither cell cycle arrest nor massive induction of apoptosis. However, more detailed analyses revealed that dampened apoptotic processes were taking place in GABPa/b−overexpressing cells, starting with a prominent activation of caspase-12. Interestingly, activation of downstream effector caspases was rather suppressed explaining a weak increase of apoptotic cells in GABPa/b overexpressing cultures. This effect suggests that the activation of caspase-12 by elevated amounts of exogenous GABPa/b reflects the normal physiological mechanism of caspase-12 regulation.
The transcriptional coactivator Bob1 promotes the development of follicular T helper cells via Bcl6

Follicular T helper (Tfh) cells are key regulators of the germinal center reaction and long-term humoral immunity. Tfh cell differentiation requires the sustained expression of the transcriptional repressor Bcl6; however, its regulation in CD4\(^+\) T cells is incompletely understood. Here, we report that the transcriptional coactivator Bob1, encoded by the Pou2af1 gene, promotes Bcl6 expression and Tfh cell development. We found that Bob1 together with the octamer transcription factors Oct1/Oct2 can directly bind to and transactivate the Bcl6 and Btla promoters. Mixed bone marrow chimeras revealed that Bob1 is required for the expression of normal levels of Bcl6 and BTLA, thereby controlling the pool size and composition of the Tfh compartment in a T cell-intrinsic manner. Our data indicate that T cell-expressed Bob1 is directly involved in Tfh cell differentiation and required for mounting normal T cell-dependent B-cell responses.
B-cells of the rheumatoid synovial tissue are a constant part of and, in some histopathological subtypes, the dominant population of the inflammatory infiltrate, located in the region of tissue destruction. The pattern of B-cell distribution and the relationship to the corresponding antigen-presenting cells (follicular dendritic reticulum cells: FDCs) show a great variety. B-cells may exhibit (i) a follicular organization forming secondary follicles; (ii) follicle-like patterns with irregularly formed FDC networks, and (iii) a diffuse pattern of isolated FDCs. Molecular analysis of immunoglobulin VH and VL genes from human synovial B-cell hybridomas and synovial tissue demonstrates somatic mutations due to antigen activation. The FDC formations in the synovial tissue may therefore serve as an environment for B-cell maturation, which is involved in the generation of autoantibodies. An autoantibody is defined as "pathogenic" if it fulfills the Witebsky-Rose-Koch criteria for classical autoimmune diseases: definition of the autoantibody; induction of the disease by transfer of the autoantibody; and isolation of the autoantibody from the disease-specific lesion. B-cells from rheumatoid synovial tissue show specificity for FcIgG, type II collagen, COMP, sDNA, tetanus toxoid, mitochondrial antigens (M2), filaggrin and bacterial HSPs. The contributions of these antigens to the pathogenesis of RA are still hypothetical. A possible contribution could derive from crossreactivity and epitope mimicry: due to crossreaction, an antibody directed originally against a foreign infectious agent could react with epitopes from articular tissues, perpetuating the local inflammatory process. The characteristic distribution pattern, the localisation within the area of tissue destruction, the hypermutated IgVH and IgVL genes, and their exclusive function to recognize conformation-dependent antigens suggest a central role for B-cells in the inflammatory process of rheumatoid arthritis. Therefore, the analysis of synovial B-cell hybridomas and experimental expression of synovial IgVH and IgVL genes will help to characterise the antigens responsible for the pathogenesis of rheumatoid arthritis. In the present study 55 IgVH genes amplified from 3 different anatomical regions of a RA patient were analysed adding further information on synovial B-cell maturation and recirculation in RA. This analysis demonstrated somatically mutated IgVh genes in all different regions with amino acid deletions and mixed IgVh molecules, suggesting the existence of a novel pathway to generate (auto)antibody specificities. The comparison of amino acid sequences of amplified genes belonging to the VH1 family (with predominantly the same germline counterpart) exhibited a strong homology, indicating an apparently conserved mutational pattern. This suggests that the number of antigens activating B-cells in the different locations is restricted. The most striking result was the finding of clonally related sequences in different anatomical regions indicating a recirculation of activated B-cells between the different affected joints. Also in the present study a synovial B-cell hybridoma was analyzed for its specific recognition of cartilage antigens. A heptameric peptide of cartilage oligomeric protein (COMP) could be defined as the target structure. The IgVH-gene (IgHV4-59*01) of the IgG2l hybridoma has somatically mutated genes with high R/S values in the CDR regions (9:2). Thus, indicating that this hybridoma originates from a synovial B-cell which has been antigen activated/selected for its affinity. To analyse the presence of the clonotypic IgHV4-59*01 sequences in other cases of RA and osteoarthritis (OA) synovitis, primers specific for the CDR3 rearrangement of this hybridoma were used. The clonotypic and clone related sequences (98 per cent ± 1 per cent homology) could only be detected in synovitis of RA cases but not in OA cases indicating that this B-cell is specific to RA synovitis. The identified heptameric peptide of COMP was used in a peptide ELISA to analyse whether there is a specific binding in RA serum samples. Serum samples (IgG) from RA patients (n=22) showed a significant higher efficiency to the COMP heptamer than the OA sera (n=24) and the age matched healthy controls (n=20) (for both p<1x10-4, Students t-test). The specificity of this B-cell hybridoma may therefore be defined as RA specific. Since COMP is restricted to cartilage and tendons which are organs specifically affected in RA this COMP specific autoantibody represents the first organ specific autoantibody in RA. The IgG2 COMP specific autoantibody with somatically mutated IgVH genes is different from germline encoded, antigen clearing IgM autoantibodies and may therefore be directly involved as an "arthritogenic autoantibody" in cartilage and tendons destruction by complement activation.
Diffuse großzellige B-Zell Lymphome stellen weltweit die größte Gruppe maligner B-Zell Non-Hodgkin-Lymphome dar und umfassen eine biologisch, genetisch und klinisch heterogene Gruppe lymphoider Tumoren, die als Hauptmerkmal große transformierte B-Lymphozyten mit vesikulären Kernen und prominenten Nukleoli aufweisen. Nur ungefähr 40% der Patienten zeigen ein Ansprechen auf eine konventionelle Chemotherapie. Um von einer präziseren Prognose profitieren zu können, ist eine reproduzierbare Klassifizierung dieser „inhomogenen Entität“ nicht nur für die Betroffenen wünschenswert. Dadurch könnten Krankheitsverläufe genauer vorhergesagt und die Therapie optimiert werden. Während akute Leukämien häufig mit einer Translokation assoziiert sind, zeigt sich bei B-Zell-Lymphomen ein weitaus komplexeres Bild an zytogenetischen Aberrationen. Diese werden einerseits mit der Etablierung des malignen Phänotyps, andererseits mit der Tumorprogression und der klonalen Evolution eines Tumors in Verbindung gebracht. Obwohl die meisten Neoplasien rekurrente und Tumor-spezifische klonale chromosomale Aberrationen aufweisen, ist es noch nicht gelungen, durch Etablieren genetischer „Marker“ eine zuverlässige Aussage über Verlauf und Ansprechen und damit über die Prognose dieser Erkrankung zu machen. Solche Aussagen sind bis dato nur auf Basis allgemeiner klinischer Parameter wie dem Allgemeinzustand der Patienten und der Höhe der Lactat-Dehydrogenase (LDH) im Serum gesichert möglich, die neben anderen Parametern im „International Prognostic Index“ zusammengefasst sind. Zytogenetische, molekulargenetische und in den letzten Jahren auch Hochdurchsatz-Untersuchungen, wie z.B. die Mikroarray-Technologie, haben bereits zu einem besseren Verständnis dieser molekularen Unterschiede geführt. Die WHO-Klassifikation stellt im Hinblick auf die Definition ‚biologischer’ Tumorgruppen einen deutlichen Fortschritt dar. Diese neueren Untersuchungen zeigen auf der Basis des Genexpressionsprofils von diffusen großzelligen B-Zell-Lymphomen die mögliche Unterteilbarkeit dieser diagnostischen Kategorie anhand zweier unterschiedlicher Phänotypen: GC-DLBCL zeigen eine molekulare Signatur, die vergleichbar mit normalen Keimzentrumszellen ist. Die andere Gruppe (ABC-DLBCL bzw. non-GC-DLBCL) entsteht aus Zellen, die die Keimzentrumsreaktion bereits durchlaufen haben. GC- und ABC-DLBCL-Patienten weisen bei einer Anthracyclin-haltigen Chemotherapie (z. B. CHOP) in retrospektiven klinischen Studien einen deutlichen Unterschied in der 5-Jahres-Überlebensrate auf (etwa 60% für GC-DLBCL und 35% für ABC-DLBCL). Somit scheint die Annahme, dass mindestens zwei Entitäten vorliegen, gerechtfertigt. In der vorliegenden Studie konnten das zytogenetische Aberrationsspektrum mittels eines Algorithmus anhand des Genexpressionsprofiles mit zwei verschiedenen Subgruppen – den GC-DLBCL und den non-GC-DLBCL - korreliert werden. Es entstand die bisher umfangreichste zytogenetische Charakterisierung dieser postulierten Phänotypen. Es konnte gezeigt werden, dass GC-DLBCL, die durch die Translokation t(14;18) charakterisiert sind, häufiger Zugewinne bei Chromosom 7 aufweisen, während non-GC-DLBCL mit dem Vorliegen einer Trisomie 3 und Zugewinnen bei 3p und 3q assoziiert sind. Zwei Modelle könnten eine Erklärung für die Assoziation genomischer Instabilitäten mit den unterschiedlichen Genexpressionsprofilen sein. Einerseits könnte eine gewisse Region für die Kodierung eines Schlüsselregulatorgenes verantwortlich sein, welches die Zellbiologie und damit die Genexpression verändert. Andererseits könnten die Subgruppen der DLBCL auf verschiedenem Wege entstehen und somit unterschiedliche Ereignisse für die verschiedenen Aberrationen verantwortlich sein. Es gilt nun, einerseits diese Mechanismen, die zu einer veränderten Genexpression beitragen, aufzudecken und andererseits ein diagnostisches Vorgehen zu etablieren, bei dem beispielsweise nach dem erfolgten Nachweis von Index-Aberrationen eine bestimmte molekulare Signatur überprüft wird. Es ist anzunehmen, dass molekulare Analysen in absehbarer Zeit vermehrt Einzug in den klinischen Alltag halten und therapeutische Entscheidungen mit beeinflussen werden. In Zukunft wird anhand der Expression von Schlüsselgenen die Zuordnung eines Falles in diagnostische Untergruppen erfolgen. Außerdem könnte durch Peptidblockade dieser Schlüsselgene eine zusätzliche Therapieoption bestehen – eine Vermutung, die weitere Studien erforderlich macht.
Dysregulation of the apoptotic pathway is widely recognized as a key step in lymphomagenesis. Notably, LITAF was initially identified as a p53-inducible gene, subsequently implicated as a tumor suppressor. Our previous study also showed LITAF to be methylated in 89.5% B-NHL samples. Conversely, deregulated expression of BCL6 is a pathogenic event in many lymphomas. Interestingly, our study found an oppositional expression of LITAF and BCL6 in B-NHL. In addition, LITAF was recently identified as a novel target gene of BCL6. Therefore, we sought to explore the feedback loop between LITAF and BCL6 in B-NHL. Here, our data for the first time show that LITAF can repress expression of BCL6 by binding to Region A (−87 to +65) containing a putative LITAF-binding motif (CTCCC) within the BCL6 promoter. Furthermore, the regulation of BCL6 targets (PRDM1 or c-Myc) by LITAF may be associated with B-cell differentiation. Results also demonstrate that ectopic expression of LITAF induces cell apoptosis, activated by releasing cytochrome c, cleaving PARP and caspase 3 in B-NHL cells whereas knockdown of LITAF robustly protected cells from apoptosis. Interestingly, BCL6, in turn, could reverse cell apoptosis mediated by LITAF. Collectively, our findings provide a novel apoptotic regulatory pathway in which LITAF, as a transcription factor, inhibits the expression of BCL6, which leads to activation of the intrinsic mitochondrial pathway and tumor apoptosis. Our study is expected to provide a possible biomarker as well as a target for clinical therapies to promote tumor cell apoptosis.
Mesenteric lymph nodes (mLNs) are sentinel sites of enteral immunosurveillance and immune homeostasis. Immune cells from the gastrointestinal tract (GIT) are constantly recruited to the mLNs in steady-state and under inflammatory conditions resulting in the induction of tolerance and immune cells activation, respectively. Surgical dissection and transplantation of lymph nodes (LN) is a technique that has supported seminal work to study LN function and is useful to investigate resident stromal and endothelial cell biology and their cellular interactions in experimental disease models. Here, we provide a detailed protocol of syngeneic mLN transplantation and report assays to analyze effective mLN engraftment in congenic recipients. Transplanted mLNs allow to study T cell activation and proliferation in preclinical mouse models. Donor mLNs proved viable and functional after surgical transplantation and regenerated blood and lymphatic vessels. Immune cells from the host completely colonized the transplanted mLNs within 7-8 weeks after the surgical intervention. After allogeneic hematopoietic cell transplantation (allo-HCT), adoptively transferred allogeneic CD4+ T cells from FVB/N (H-2q) mice homed to the transplanted mLNs in C57BL/6 (H-2b) recipients during the initiation phase of acute graft-versus-host disease (aGvHD). These CD4+ T cells retained full proliferative capacity and upregulated effector and gut homing molecules comparable to those in mLNs from unmanipulated wild-type recipients. Wild type mLNs transplanted into MHCII deficient syngeneic hosts sufficed to activate alloreactive T cells upon allogeneic hematopoietic cell transplantation, even in the absence of MHCII+ CD11c+ myeloid cells. These data support that orthotopically transplanted mLNs maintain physiological functions after transplantation. The technique of LN transplantation can be applied to study migratory and resident cell compartment interactions in mLNs as well as immune reactions from and to the gut under inflammatory and non-inflammatory conditions.
Purpose
In cancer of unknown primary (CUP), positron emission tomography/computed tomography (PET/CT) with the glucose analog [\(^{18}\)F]FDG represents the standard imaging approach for localization of the malignant primary. Frequently, however, [\(^{18}\)F]FDG PET/CT cannot precisely distinguish between small occult tumors and chronic inflammation, especially in Waldeyer’s tonsillar ring. To improve the accuracy for detecting primary tumors in the Waldeyer’s tonsillar ring, the novel PET tracer [\(^{68}\)Ga]Ga-FAPI-4 for specific imaging of fibroblast activation protein (FAP) expression was used as a more specific target for cancer imaging.
Methods
Eight patients with suspicion of a malignant tumor in Waldeyer’s tonsillar ring or a CUP syndrome were examined. PET/CT scans with [\(^{18}\)F]-FDG and [\(^{68}\)Ga]Ga-FAPI-4 were performed for pre-operative tumor localization. After surgical resection, histopathological and immunohistochemical results were compared to PET/CT findings.
Results
Histopathology revealed a palatine or lingual tonsil carcinoma in all patients. In case of lymph node metastases smaller than 7 mm in size, the [\(^{18}\)F]FDG PET/CT detection rate of cervical lymph node metastases was higher than that of [\(^{68}\)Ga]FAPI PET/CT, while both tracers identified the primary tumors in all eight cases. The size of the primary and the lymph node metastases was directly correlated to the respective FAP expression, as detected by immunohistochemistry. The mean SUVmax for the primary tumors was 21.29 ± 7.97 for \(^{18}\)F-FDG and 16.06 ± 6.29 for \(^{68}\)Ga-FAPI, respectively (p = 0.2). The mean SUVmax for the healthy contralateral tonsils was 8.38 ± 2.45 for [\(^{18}\)F]FDG and 3.55 ± 0.47 for [\(^{68}\)Ga]FAPI (p < 0.001). The SUVmax ratio of [68Ga]FAPI was significantly different from [\(^{18}\)F] FDG (p = 0.03). Mean TBRmax for the [\(^{68}\)Ga]Ga-FAPI-4 tracer was markedly higher in comparison to [\(^{18}\)F]FDG (10.90 vs. 4.11).
Conclusion
Non-invasive imaging of FAP expression by [\(^{68}\)Ga]FAPI PET/CT resulted in a better visual detection of the malignant primary in CUP, as compared to [\(^{18}\)F]FDG imaging. However, the detection rate of lymph node metastases was inferior, presumably due to low FAP expression in small metastases. Nevertheless, by offering a detection method for primary tumors with the potential of lower false positive rates and thus avoiding biopsies, patients with CUP syndrome may benefit from [\(^{68}\)Ga]FAPI PET/CT imaging.
In lymphocytes, the three NFAT factors NFATc1 (also designated as NFAT2), NFATc2 (NFAT1), and NFATc3 (NFAT4 or NFATx) are expressed and are the targets of immune receptor signals, which lead to a rapid rise of intracellular Ca++, the activation of phosphatase calcineurin, and to the activation of cytosolic NFATc proteins. In addition to rapid activation of NFAT factors, immune receptor signals lead to accumulation of the short NFATc1/αA isoform in lymphocytes which controls their proliferation and survival. In this mini-review, we summarize our current knowledge on the structure and transcription of the Nfatc1 gene in lymphocytes, which is controlled by two promoters, two poly A addition sites and a remote downstream enhancer. The Nfatc1 gene resembles numerous primary response genes (PRGs) induced by LPS in macrophages. Similar to the PRG promoters, the Nfatc1 promoter region is organized in CpG islands, forms DNase I hypersensitive sites, and is marked by histone tail modifications before induction. By studying gene induction in lymphocytes in detail, it will be important to elucidate whether the properties of the Nfatc1 induction are not only typical for the Nfatc1 gene but also for other transcription factor genes expressed in lymphocytes.
In effector T and B cells immune receptor signals induce within minutes a rise of intracellular Ca++, the activation of the phosphatase calcineurin and the translocation of NFAT transcription factors from cytosol to nucleus. In addition to this first wave of NFAT activation, in a second step the occurrence of NFATc1/αA, a short isoform of NFATc1, is strongly induced. Upon primary stimulation of lymphocytes the induction of NFATc1/αA takes place during the G1 phase of cell cycle. Due to an auto-regulatory feedback circuit high levels of NFATc1/αA are kept constant during persistent immune receptor stimulation. Contrary to NFATc2 and further NFATc proteins which dampen lymphocyte proliferation, induce anergy and enhance activation induced cell death (AICD), NFATc1/αA supports antigenmediated proliferation and protects lymphocytes against rapid AICD. Whereas high concentrations of NFATc1/αA can also lead to apoptosis, in collaboration with NF-κB-inducing co-stimulatory signals they support the survival of mature lymphocytes in late phases after their activation. However, if dysregulated, NFATc1/αA appears to contribute to lymphoma genesis and – as we assume – to further disorders of the lymphoid system. While the molecular details of NFATc1/αA action and its contribution to lymphoid disorders have to be investigated, NFATc1/αA differs in its generation and function markedly from all the other NFAT proteins which are expressed in lymphoid cells. Therefore, it represents a prime target for causal therapies of immune disorders in future.
Active vitamin D (1,25(OH)2D3) is known to exert direct anti-cancer actions on various malignant tissues through binding to the vitamin D receptor (VDR). These effects have been demonstrated in breast, prostate, renal and thyroid cancers, which all have a high propensity to metastasise to bone. In addition, there is evidence that vitamin D catabolism via 24-hydroxylase (CYP24A1) is altered in tumour cells, thus, reducing local active vitamin D levels in cancer cells. The aim of this study was to assess VDR and CYP24A1 expression in various types of bone metastases by using immunohistochemistry. Overall, a high total VDR protein expression was detected in 59% of cases (39/66). There was a non-significant trend of high-grade tumours towards the low nuclear VDR expression (p = 0.07). Notably, patients with further distant metastases had a reduced nuclear VDR expression (p = 0.03). Furthermore, a high CYP24A1 expression was detected in 59% (39/66) of bone metastases. There was a significant positive correlation between nuclear VDR and CYP24A1 expression (p = 0.001). Collectively, the VDR and CYP24A1 were widely expressed in a multitude of bone metastases, pointing to a potential role of vitamin D signalling in cancer progression. This is of high clinical relevance, as vitamin D deficiency is frequent in patients with bone metastases.
In der vorliegenden Studie haben wir mit Hilfe von SHARP-Screening, einer 16S-rRNA-basierten Heterogenitäts- und phylogenetischen Analyse, die mikrobielle Diversität in entzündlichen Lymphadenitiden ohne vorherige Kenntnis der jeweiligen Erreger an einer Serie von 15 Lymphknoten untersucht. Die Methode wurde erstmals auf diese Fragestellung angewandt. Sie konnte für die Verwendung von paraffineingebettetem Gewebe adaptiert werden, so dass auch Gewebeproben analysiert werden konnten, von denen kein Gefriermaterial zur Verfügung stand und die in Routineverfahren eingebettet und nach Standardmethoden gefärbt wurden. SHARP-Screening beinhaltet zwei komplementäre Schritte: Zuerst erfolgte die Erstellung einer Genbank aller bakteriellen Gene aus der gesamten extrahierten DNA des analysierten Gewebes durch gezielte Amplifikation des 16S-rRNA-Gens mittels universeller eubakterieller Primer. Als zweiter Schritt wurde nach der Transformation mittels Analyse des Restriktionsfragmentlängenpolymorphismus die Selektion der jeweiligen unterschiedlichen Phylotypen der enthaltenen 16S-rRNA-Gene durchgeführt (insgesamt 400). Nach der Sequenzierung wurden die 16S-rRNA-Gene durch den Vergleich mit bekannten bakteriellen Sequenzen mit Hilfe des „Basic-Local-Alignment-Search-Tool“ (BLAST) identifiziert. SHARP-Screening hat sich als geeignete Methode zur Analyse der gesamten, in einer Gewebeprobe enthaltenen bakteriellen Flora erwiesen. Dabei wurden zum Teil andere Erreger gefunden, als aus dem histologischen Bild vermutet wurden. So konnte zum Beispiel mit dem Nachweis von Gluconacetobacter sacchari, als potentieller Erreger einer septischen Granulomatose, eine alternative Differentialdiagnose zur histologisch vermuteten Katzenkratzkrankheit aufgezeigt werden. Darüber hinaus konnten auch gleichartige histologische Bilder bei dem gleichen identifizierten Erreger beobachtet werden. Zum Beispiel konnten im Zusammenhang mit dem Auftreten von Ödemen, Nekrosen, granulomatös eitrigen Veränderungen und einer ausgeprägten Sinus-histiozytose im Lymphknotengewebe immer wieder Comamonadaceae bzw. Janthinobacterium nachgewiesen werden. Oft zeigte sich nicht ein einzelner Erreger der Lymphadenitis, sondern ein ganzes Spektrum, wobei aus dem Vorhandensein der 16S-rRNA nicht auf das Vorhandensein vitaler Erreger geschlossen werden kann. Dennoch erlaubt die Häufigkeit der entsprechenden Klone eine semiquantitative Abschätzung der Bedeutung des jeweiligen Erregers. So wies SHARP-Screening auch Homologien zu Paracoccus yeeii nach. Eine Spezies, die mit klassischen Methoden häufig übersehen wird, die in Lymphknoten jedoch eine pathogene Rolle spielen kann. Im Zusammenhang mit dem histologischen Verdacht auf ein Malignom wurden in einigen Fällen Streptomyces, Roseomonas gilardii rosea und Stenotrophomonas maltophila nachgewiesen, die auch in der Literatur häufig bei immunsupprimierten Patienten vorkommen. Bei dem Verdacht auf ein Lymphgranuloma venerum wurde eine Cyanobacterium-Spezies detektiert, die es nach Literaturangaben Chlamydia trachomatis erst möglich macht, den eigenen Aminosäurestoffwechsel zu betreiben. Insgesamt dürften vom SHARP-Screening noch weitere tiefgreifende Erkenntnisse der bakteriellen Diversität und kausaler Erregerassoziationen in Erkrankungen des lymphatischen Systems zu erwarten sein.
B-Zell-Lymphome vom mukosa-assoziierten lymphatischen Gewebe (MALT)-Typ sind die größte Gruppe der extranodalen Lymphome. Sie enstehen vor dem Hintergrund einer chronischen Entzündung, wie etwa einer Helicobacter pylori-assoziierten Gastritis im Magen. Die Mechanismen der Lymphomgenese sind weitgehend unverstanden. Der Befund, dass die malignen B-Zellen der MALT-Typ Lymphome autoreaktiv sind und durch Antigen oder T-Zell-vermittelte Signale stimuliert werden, weist auf ein mögliches Ver-sagen der T-Zell-Kontrolle hin. Zur Prüfung dieser Hypothese wurden in der vorliegenden Arbeit sowohl tumor-infiltrierende T-Zellen als auch maligne B-Zellen aus MALT-Typ Lymphomen untersucht. Mit Hilfe von Expressionsanalysen der Vb -Kette des T-Zell-Rezeptors (TCR) wurden antigen-induzierte klonale Expansionen in den tumor-infiltrierenden T-Zellen nachgewiesen. Weiterhin wurden ähnliche antigen-erkennende Regionen (CDR3) des TCR in tumor-infiltrierenden T-Zellen aus zwei verschiedenen MALT-Typ Lymphomen gefunden. Diese Ergebnisse charakterisieren die T-Zellen als funktionell und auch als potenziell tumor-reaktiv. Um mögliche Defekte bei malignen B-Zellen zu untersuchen, wurde eine in vitro T/B-Zell-Kokultur entwickelt. Damit war es möglich, die Interaktion des apoptose-induzierenden Oberflächenmoleküls FasL auf aktivierten T-Zellen mit dem entsprechenden Todesrezeptor Fas auf malignen B-Zellen zu untersuchen. Drei von sieben MALT-Typ Lymphomen und vier von fünf DLBL erwiesen sich als resistent gegen FasL-vermittelte Apoptose. Meine Untersuchungen deuten darauf hin, dass hierfür eine mutationsbedingte funktionelle Inaktivierung des Fas-Rezeptors verantwortlich ist. In Fas-Transkripten aller untersuchten malignen B-Zellen wurden insgesamt 14 verschiedene Punktmutationen gefunden, die zu Aminosäureaustauschen bei der Translation führen. Zehn dieser Mutationen waren mit der Apoptose-Resistenz maligner B-Zellen assoziiert. Durch ergänzende Untersuchungen konnten alternative Mechanismen der Apoptose-Resistenz wie etwa reduzierte Fas-Expression, Produktion von löslichem Fas (sFas) oder Störungen in der Fas-Signalkaskade weitgehend ausgeschlossen werden. Aus diesen Ergebnissen lässt sich folgender Schluss ziehen: Resistenz gegen FasL/Fas-vermittelte Apoptose ist ein Mechanismus der frühen MALT-Typ Lymphomgenese und möglicherweise auf bestimmte Fas-Mutationen zurückzuführen. Durch Apoptose-Resistenz entkommen die malignen B-Zellen der MALT-Typ Lymphome der vorhandenen T-Zell-Kontrolle. Gleichzeitig nehmen sie die parakrin angebotene T-Zell-Hilfe solange in Anspruch, bis sie völlige Autonomie erreicht haben. Durch das abnorm verlängerte Überleben der B-Zellen steigt dann die Wahrscheinlichkeit, weitere Aberrationen wie etwa die in 50 Prozent aller Fälle auftretende Chromosomentranslokation t(11;18)(q21;q21) zu erwerben.
Delayed and limited administration of the JAKinib tofacitinib mitigates chronic DSS-induced colitis

In inflammatory bowel disease, dysregulated T cells express pro-inflammatory cytokines. Using a chronic azoxymethane (AOM)/dextran sulfate sodium (DSS)-induced colitis model resembling ulcerative colitis, we evaluated whether and when treatment with the Janus kinase (JAK) inhibitor tofacitinib could be curative. Comparing the treatment with two and three cycles of tofacitinib medication in drinking water – intermittently with DSS induction – revealed that two cycles were not only sufficient but also superior over the 3-x regimen. The two cycles of the 2-x protocol paralleled the second and third cycles of the longer protocol. T cells were less able to express interferon gamma (IFN-γ) and the serum levels of IFN-γ, interleukin (IL)-2, IL-6, IL-17, and tumor necrosis factor (TNF) were significantly reduced in sera, while those of IL-10 and IL-22 increased under the 2-x protocol. Likewise, the frequency and effector phenotype of regulatory T cells (Tregs) increased. This was accompanied by normal weight gain, controlled clinical scores, and restored stool consistency. The general and histologic appearance of the colons revealed healing and tissue intactness. Importantly, two phases of tofacitinib medication completely prevented AOM-incited pseudopolyps and the hyper-proliferation of epithelia, which was in contrast to the 3-x regimen. This implies that the initial IBD-induced cytokine expression is not necessarily harmful as long as inflammatory signaling can later be suppressed and that time-restricted treatment allows for anti-inflammatory and tissue-healing cytokine activities.
T-cell lymphomas are highly heterogeneous and their prognosis is poor under the currently available therapies. Enhancers of zeste homologue 1 and 2 (EZH1/2) are histone H3 lysine-27 trimethyltransferases (H3K27me3). Despite the rapid development of new drugs inhibiting EZH2 and/or EZH1, the molecular interplay of these proteins and the impact on disease progression and prognosis of patients with T-cell lymphomas remains insufficiently understood. In this study, EZH1/2 mutation status was evaluated in 33 monomorphic epitheliotropic intestinal T-cell lymphomas by next generation sequencing and EZH1/2 and H3K27me3 protein expression levels were detected by immunohistochemistry in 46 T-cell lymphomas. Correlations with clinicopathologic features were analyzed and survival curves generated. No EZH1 mutations and one (3%) EZH2 missense mutation were identified. In univariable analysis, high EZH1 expression was associated with an improved overall survival (OS) and progression-free survival (PFS) whereas high EZH2 and H3K27me3 expression were associated with poorer OS and PFS. Multivariable analysis revealed EZH1 (hazard ratio (HR) = 0.183; 95% confidence interval (CI): 0.044–0.767; p = 0.020;) and EZH2 (HR = 8.245; 95% CI: 1.898–35.826; p = 0.005) to be independent, divergent prognostic markers for OS. In conclusion, EZH1/2 protein expression had opposing effects on the prognosis of T-cell lymphoma patients.
Die Expression der Hämatopoetischen Progenitor Kinase 1 (HPK1), einem Mitglied der Familie der „Germinal Centre“ Kinasen, ist im adulten Organismus auf die Zellen des hämatopoetischen Systems beschränkt. Die HPK1 wurde ursprünglich als ein Aktivator des JNK-Signalübertragungsweges beschrieben [Hu et al., 1996; Kiefer et al., 1996], und kürzlich wurde eine transiente Aktivierung der HPK1 nach TZR-Stimulation nachgewiesen. Auch wurde eine Assoziation der HPK1 mit dem Linker aktivierter T-Zellen (LAT) und den Adaptorproteinen Nck, Crk, Gads, Grb2, Grap, CrkL sowie SLP-76 gezeigt. Für die Aktivierung der nach TZR-Stimulation in den Lipid-Rafts lokalisierten HPK1 sind sowohl Lck als auch ZAP-70 notwendig [Liou et al., 2000; Liu et al., 2000a; Ling et al., 2001]. Diese Daten legen eine mögliche Funktion von HPK1 bei der TZR-vermittelten Signalübertragung nahe. Trotzdem konnte bisher eine physiologische Rolle der HPK1 im Rahmen der Immunrezeptor-Signalübertragung nicht nachgewiesen werden. In der vorliegenden Arbeit wird dargestellt, dass eine wichtige Funktion der HPK1 in T-Zellen nach TZR-Stimulation, die durch die Förderung des Aktivierungs-induzierten Zelltodes (AICD) vermittelt wird, in der Kontrolle der Termination der Immunantwort und damit Homöostase des Immunsystems besteht. Dies wurde durch retrovirale Überexpression der wildtypischen (wt) HPK1 in murinen CD4+ T-Zellen nachgewiesen, in denen die HPK1 zu einem Anstieg der spontanen und antiCD3-vermittelten Apoptose sowie zu einer gesteigerten Expression des Fas-Liganden (FasL oder auch CD95L) führte. Die Expression einer HPK1-„antisense“ (AS)-RNA in CD4+ T-Zellen bewirkte dagegen eine schwache, jedoch signifikant nachweisbare Hemmung der Apoptose und FasL-Expression. Die Apoptose-Hemmung durch die HPK1-AS-RNA war besonders stark in H2O2-stimulierten EL-4 T-Zellen ausgeprägt, in denen die Überexpression der wt HPK1 den durch reaktive Sauerstoffmetabolite (ROS) induzierten Zelltod verstärkte. Aus diesen Daten folgt, dass die HPK1 die T-Zell-Apoptose reguliert. In H2O2-stimulierten EL-4 T-Zellen führt die HPK1-Expression zu einer verstärkten und anhaltenden Aktivierung der c-Jun N-terminalen Kinase (JNK), die wahrscheinlich an der HPK1-vermittelten Apoptoseinduktion beteiligt ist. Unter den gleichen Bedingungen konnte eine schnelle Spaltung der HPK1 beobachtet werden. Die Überexpression der N- oder C-terminalen Spaltprodukte in CD4+ T-Zellen führte - wie die der Gesamt-HPK1 - zu einem Anstieg des AICD. In Übereinstimmung mit publizierten Daten konnten wir eine Hemmung der NFkB-Aktivität durch das C-terminale HPK1-Peptid nachweisen, das die IkBalpha-Degradation inhibiert. Die erzielten Ergebnisse führten uns in ihrer Gesamtheit zu folgendem Modell: während der Initiationsphase der T-Zell-Stimulierung werden nach schneller, transienter HPK1-Aktivierung pro- und anti-apoptotische Signale durch den JNK- und NFkB-Signalübertragungsweg vermittelt. Durch die Akkumulation der C-terminalen HPK1-Spaltprodukte kommt es später zur Inhibierung der NFkB-Aktivität und damit zu einer Verschiebung des Gleichgewichts zwischen den Überlebens- und Apoptose-stimulierenden Signalen zugunsten des AICD. Allerdings gibt es sicherlich weitere Faktoren und Signalwege, die an der HPK1-vermittelten Kontrolle der T-Zell-Apoptose beteiligt sind und von deren Untersuchung ein detaillierteres Verständnis der HPK1-Physiologie erwartet wird. Die Nukleären Faktoren aktivierter T-Zellen (NFAT´s) gehören zu einer Familie von Transkriptionsfaktoren, denen eine konservierte, ca. 300 Aminosäuren (aa) große DNA-Bindedomäne und eine Calcineurin-Bindedomäne gemeinsam ist. NFATc (auch NFATc1 oder NFAT2 genannt) und NFATp (NFATc2 oder NFAT1) werden in peripheren T-Zellen stark exprimiert und kontrollieren deren Effektorfunktionen u.a. über die Expression von IL-2, IL-3, IL-4, IL-5, IFNgamma und weiterer Lymphokine. Weitere von den NFAT´s kontrollierte Gene sind p21WAF1, der CD40L und der CD95L. Somit scheinen die NFAT´s bei der Zellzyklus-Kontrolle und beim AICD von T-Lymphozyten eine wichtige Rolle zu spielen. Daten unseres Labors zeigten, dass die T-Zell-Aktivierung zu einer massiven Induktion der kurzen Isoform A von NFATc innerhalb von 3-4 h führt [Chuvpilo et al., 1999b], noch vor dem Start des AICD. Dies ließ vermuten, dass sich die biologische Funktion von NFATc/A durch das Fehlen des C-terminalen Peptids von ca. 245 aa, das in allen anderen NFAT-Proteinen einschließlich der längeren Isoform NFATc/C vorhanden ist, unterscheidet. Deshalb sollte in der vorliegenden Arbeit die Induktion und Funktion von NFATc/A in murinen T-Lymphozyten untersucht werden. Es konnte gezeigt werden, dass die Infektion primärer CD4+ T-Lymphozyten mit NFATc/A-exprimierenden rekombinanten Retroviren, im Gegensatz zu der mit NFATc/C- oder NFATp-exprimierenden Retroviren, den AICD unbeeinflusst lässt. Dies deutet darauf hin, dass es durch die massive NFATc/A-Synthese nach Effektor-T-Zell-Aktivierung zur Induktion von Effektor-Funktionen kommt, ohne dass dabei die T-Zell-Apoptose beschleunigt wird. Im Gegensatz dazu üben die langen NFAT-Faktoren wie NFATc/C und NFATp eine pro-apoptotische Wirkung aus.
Background
The phosphatase chronophin (CIN/PDXP) has been shown to be an important regulator of glioma cell migration and invasion. It has two known substrates: p-Ser3-cofilin, the phosphorylated form of the actin binding protein cofilin, and pyridoxal 5′-phosphate, the active form of vitamin B6. Phosphoregulation of cofilin, among other functions, plays an important role in cell migration, whereas active vitamin B6 is a cofactor for more than one hundred enzymatic reactions. The role of CIN has yet only been examined in glioblastoma cell line models derived under serum culture conditions.
Results
We found that CIN is highly expressed in cells cultured under non-adherent, serum-free conditions that are thought to better mimic the in vivo situation. Furthermore, the substrates of CIN, p-Ser3-cofilin and active vitamin B6, were significantly reduced as compared to cell lines cultured in serum-containing medium. To further examine its molecular role we stably knocked down the CIN protein with two different shRNA hairpins in the glioblastoma cell lines NCH421k and NCH644. Both cell lines did not show any significant alterations in proliferation but expression of differentiation markers (such as GFAP or TUBB3) was increased in the knockdown cell lines. In addition, colony formation was significantly impaired in NCH644. Of note, in both cell lines CIN knockdown increased active vitamin B6 levels with vitamin B6 being known to be important for S-adenosylmethionine biosynthesis. Nevertheless, global histone and DNA methylation remained unaltered as was chemoresistance towards temozolomide. To further elucidate the role of phosphocofilin in glioblastoma cells we applied inhibitors for ROCK1/2 and LIMK1/2 to our model. LIMK- and ROCK-inhibitor treatment alone was not toxic for glioblastoma cells. However, it had profound, but antagonistic effects in NCH421k and NCH644 under chemotherapy.
Conclusion
In non-adherent glioblastoma cell lines cultured in serum-free medium, chronophin knockdown induces phenotypic changes, e.g. in colony formation and transcription, but these are highly dependent on the cellular background. The same is true for phenotypes observed after treatment with inhibitors for kinases regulating cofilin phosphorylation (ROCKs and LIMKs). Targeting the cofilin phosphorylation pathway might therefore not be a straightforward therapeutic option in glioblastoma.
Molecular-based subclassifications of breast cancer are important for identifying treatment options and stratifying the prognosis in breast cancer. This study aimed to assess the prognosis relative to disease-free survival (DFS) and overall survival (OS) in patients with triple-negative breast cancer (TNBC) and other subtypes, using a biomarker panel including cytokeratin 5 (CK5), cluster of differentiation 117 (CD117), and epidermal growth factor receptor (EGFR). This cohort–case study included histologically confirmed breast carcinomas as cohort arm. From a total of 894 patients, 572 patients with early breast cancer, sufficient clinical data, and archived tumor tissue were included. Using the immunohistochemical markers CK5, CD117, and EGFR, two subgroups were formed: one with all three biomarkers negative (TBN) and one with at least one of those three biomarkers positive (non-TBN). There were significant differences between the two biomarker subgroups (TBN versus non-TBN) in TNBC for DFS (p = 0.04) and OS (p = 0.02), with higher survival rates (DFS and OS) in the non-TBN subgroup. In this study, we found the non-TBN subgroup of TNBC lesions with at least one positive biomarker of CK5, CD117, and/or EGFR, to be associated with longer DFS and OS.
Aminoacyl‐tRNA synthetases (ARSs) catalyze the first step of protein biosynthesis (canonical function) and have additional (non‐canonical) functions outside of translation. Bi‐allelic pathogenic variants in genes encoding ARSs are associated with various recessive mitochondrial and multisystem disorders. We describe here a multisystem clinical phenotype based on bi‐allelic mutations in the two genes (FARSA, FARSB) encoding distinct subunits for tetrameric cytosolic phenylalanyl‐tRNA synthetase (FARS1). Interstitial lung disease with cholesterol pneumonitis on histology emerged as an early characteristic feature and significantly determined disease burden. Additional clinical characteristics of the patients included neurological findings, liver dysfunction, and connective tissue, muscular and vascular abnormalities. Structural modeling of newly identified missense mutations in the alpha subunit of FARS1, FARSA, showed exclusive mapping to the enzyme's conserved catalytic domain. Patient‐derived mutant cells displayed compromised aminoacylation activity in two cases, while remaining unaffected in another. Collectively, these findings expand current knowledge about the human ARS disease spectrum and support a loss of canonical and non‐canonical function in FARS1‐associated recessive disease.
Eine grundlegende Aufgabe unseres Immunsystems ist der Schutz unseres Organismus durch die Abwehr von Erregern. Im Zentrum jeder Immunantwort liegt die Unterscheidung zwischen dem „Selbst“ (der Erkennung körpereigener Antigene) und dem „Nicht-Selbst“ (der Erkennung körperfremder Antigene). Ein wichtiger Faktor in der Aufrechterhaltung der zentralen Toleranz ist das AIRE-Protein. In der Thymusmedulla, dem Ort der stärksten AIRE-Expression, erfolgt die sog. negative Selektion der heranreifenden autoreaktiven T-Zellen. Es wird angenommen, dass AIRE durch Regulation der Präsentation von Selbst-Antigenen in mTECs und DCs die Thymozyten auf Autoreaktivität kontrolliert und potentielle Auslöser einer Autoimmunkrankheit eliminiert. Thymome sind epitheliale Tumoren des Thymus, die häufig mit Autoimmunphänomenen, insbesondere paraneoplastischer Myasthenia gravis, einhergehen. Da bei Thymomen meist ein vollständiger Verlust von AIRE sowohl auf Transkriptions- als auch Proteinebene vorliegt, lag es nahe, einen Zusammenhang mit dem Auftreten Thymom-assoziierter Autoimmunerkrankungen zu vermuten. Thymome wurden initial in einer histogenetischen Klassifikation aufgrund morphologischer Kriterien in „medulläre“ und „corticale“ Typen unterteilt. Dieses Konzept sollte in der vorgelegten Arbeit überprüft werden. Durch Kombination verschiedener Gen-Expressions-Datensätze wurden „medulläre Thymusgene“ identifiziert und in 3 Gruppen (Gene ohne Beeinflussung durch AIRE bzw. Gene mit positiver bzw. negativer Regulation durch AIRE) unterteilt. Unter den hier identifizierten, durch AIRE positiv regulierten Genen fanden sich zahlreiche Gene mit potentieller Bedeutung für die Immunregulation bzw. die Entstehung experimenteller und humaner Autoimmunerkrankungen. Eine Analyse verschiedener Thymom-Subtypen ergab weder eine erkennbare „AIRE-Verlust-Signatur“ noch eine medulläre Gensignatur in den „medullären“ Typ A Thymomen. Die erhobenen Befunde wären aber mit einem Stammzellmodell der Thymomentstehung vereinbar, nach dem die unterschiedlichen Thymom-Subtypen aus unterschiedlich determinierten Stamm- oder Progenitorzellen mit verschiedenen Reifungsblockaden hervorgehen könnten.
Follikuläre Lymphome (FL) machen etwa 25-40% der Non-Hodgkin-Lymphome aus und sind in der Regel bereits bei Diagnosestellung nicht mehr auf den Lymphknoten beschränkt, sondern systemische Erkrankungen. In jüngeren Untersuchungen konnte gezeigt werden, dass die selten diagnostizierten limitierten Stadien (Ann Arbor I und II) der Erkrankung häufig einen nur partiellen Befall der betroffenen Lymphknoten durch das Lymphom zeigen. In diesen frühen Stadien kolonisieren follikuläre Lymphome präexistente Follikel (in situ- Lymphom) und breiten sich dann offenbar auf die übrigen Follikel des Lymphknotens aus, bevor ein systemischer Befall des gesamten Organismus feststellbar ist. Ziel der vorliegenden Arbeit war es zunächst zu untersuchen, auf welchem Weg die Zellen eines Tumorklons im follikulären Lymphom die Keimzentren eines Lymphknotens kolonisieren. Dazu wurde die genetische Verwandtschaft der einzelnen Tumorsubklone untereinander anhand ihrer individuellen Mutationsmuster bestimmt. Mit Hilfe von daraus berechneten phylogenetischen Stammbäumen konnte die Ausbreitung der Subklone auf die vorbestehenden Keimzentren nachvollzogen werden. Zweitens sollte in dieser Studie der Frage nachgegangen werden, ob die Tumorsubklone auch unter dem Einfluss der Keimzentrumsumgebung stehen, die in der physiologischen B-Zell-Reifung für die enorme Vielfalt der Antikörperspezifität sorgt (Hypermutation). Anhaltende Mutationen (ongoing mutations) innerhalb eines Tumorklons würden auf einen solchen erhaltenen Einfluss der Hypermutationsmaschinerie hinweisen. Schließlich sollte untersucht werden, ob es auch in follikulären Lymphomen eine antigenabhängige B-Zell-Reifung gibt, wie sie bei der physiologischen „Optimierung“ von Antikörpern auf die korrespondierenden Antigene zu finden ist. Material und Methode: Sieben Fälle von follikulären Lymphomen von vier Patienten (davon einer mit einem und einer mit zwei Rezidiven ihrer Lymphomerkrankung) wurden morphologisch und immunhistochemisch reevaluiert. Pro Fall wurden bis zu zehn Follikel mikrodisseziert und pro Follikel die VH-Gene von bis zu zehn Subklonen sequenziert. Computerunterstützt wurden sowohl die genetische Verwandtschaft der Tumorsubklone untereinander und ihre Verteilung auf die einzelnen Follikel, als auch das Verhältnis von R- zu S- Mutationen in den verschiedenen Abschnitten des BCR-Gens und damit ein möglicher Antigen-Einfluss auf die Hypermutation analysiert. Ergebnisse: Ein FL Grad I zeigte ein deutliches Clustering von genetisch miteinander verwandten Tumorsubklonen im selben Follikel. Dennoch fand sich ein moderater interfollikulärer Austausch der Subklone. Bei morphologisch höhergradigen FL (Grad II und IIIa) nahm das Clustering deutlich ab und der interfollikuläre Austausch zu, bis im zweiten Rezidiv eines Patienten ein weitgehend diffuses Wachstum resultierte. Als Ausdruck des erhaltenen Einflusses des Keimzentrums zeigten alle Primärtumoren (FL Grad I und II) noch ongoing mutations, während bei FL in Progression keine ongoing mutations mehr feststellbar waren. Eine Häufung von R-Mutationen in den antigenbindenden Domänen des B-Zell-Rezeptors (CDR) und S-Mutationen in den strukturellen Domänen (FR) als Hinweis auf eine antigen-gesteuerte Hypermutation in den Tumorsubklonen fand sich nur in einem FL Grad I. Aus den genetischen Analysen ergaben sich aber Hinweise auf eine erhaltene Funktionalität des B-Zell-Rezeptors in allen sieben Fällen.
Bei vielen Karzinomen spielt EGFR und das KRAS-Onkogen eine wichtige Rolle in der Tumorentstehung. Da bei den seltenen Karzinomen an Kopfspeicheldrüsen sehr wenig über molekulare Mechanismen der Tumorgenese bekannt ist, war es das Ziel der Arbeit den EGFR-Signalweg zu untersuchen. Es wurden Paraffinschnitte von 43 Speicheldrüsenkarzinomen von den Typen ACC, MEC und Adeno-Ca NOS mit dem phosphorylierten EGFR-Antikörper gefärbt und mit klinisch-pathologischen Daten korreliert. Weiterhin wurde eine Mutationsanalyse der kras-Gensequenz durchgeführt. In allen Fällen war das kras-Gen vom Wildtyp. Bei der Expressionsanalyse von EGFR stellte sich heraus, dass 79% der Proben einen aktivierten EGF-Rezeptor besitzen. Statistisch signifikante Korrelationen gab es zwischen der EGFR-Expression und dem Patientenalter, dem zervikalen Lymphknotenbefall und der Tumorgröße. Der EGF-Signaltransduktionsweg ist bei den untersuchten Karzinomen der Kopfspeicheldrüsen im überwiegenden Masse aktiviert, ohne dass eine autonome Aktivierung beim KRAS-Onkogen vorliegt.
Zur Entscheidungshilfe in der Therapiefindung des Mammakarzinoms haben sich bezüglich der Indikation zur Chemotherapie neben den klinischen und histopathologischen Kriterien in den letzten Jahren vorrangig Multigentests etabliert. In der vorliegenden Arbeit wurden Zusammenhänge zwischen dem Oncotyp DX® und 18 immunhistochemischen Markern aus der Tumorbiologie für 78 Fälle hormonrezeptorpositiver, Her2/neu negativer Mammafrühkarzinome mit niedrigem Lymphknotenstatus untersucht. Es erfolgten immunhistochemische Färbungen an Microtissue-Arrays der Tumorproben. Für die Marker AMACR, Cyclin D1, p53, MDM2 und PDL1 ergab sich eine klare statistisch signifikante Korrelation zum Recurrence-Score®des Oncotyp DX® und mit Einschränkungen auch für CDK4. Die Marker p27, Bcl2 und Glut 1 erreichten ein etwas niedrigeres Signifikanzniveau in der statistischen Analyse. Der immunhistochemische Routinemarker Ki67% zeigte eine hochsignifikante Korrelation mit dem Recurrence-Score®. Hierdurch ergeben sich neue Perspektiven zur Risikostratifizierung des Mammakarzinoms, wie beispielsweise die konsekutive Entwicklung eines immunhistochemischen Scores mit prädiktivem Wert für den Recurrence-Score® mit klinischer Anwendung als Prätest oder als eigenständiges Stratifizierungstool bei Brustkrebs.
Die histologische Technik der Tissue-Microarrays ist eine sehr effiziente Methode, um eine große Anzahl auch heterogener Lymphome wie des Hodgkin-Lymphoms bei hohem Durchsatz unter homogenen Färbebedingungen zu untersuchen. Die vorliegende Arbeit konnte zeigen, dass ein Probeschnitt zur vorherigen Auswahl stanzwürdigen Tumorareals nicht nötig ist. Die so genannte Blindstanzung trug weniger als einen Prozentpunkt (0,9%) zum Verlust der auswertbaren Fälle bei. Dennoch war bei einzelnen Parametern (LMP1 und EBER) ein hoher Gewebeverlust zu beobachten. In einer Stichprobe von 2696 Stanzen waren es 24% (631 Stanzen) bedingt durch die Färbetechnik, aber auch durch unterschiedliche Vorbehandlungen und Originalfixationen des Probematerials. In dieser Studie wurden 1212 Fälle zur Immuntypisierung von Tumorzellen des klassischen und nodulär lymphozyten-prädominanten Hodgkin-Lymphoms untersucht. Die Fälle von c-HL wiesen eine häufige Expression von CD30- und CD15-Oberflächenmarkern und kaum B-Zell-Marker auf, während im NLP-HL die Expression in umgekehrter Häufigkeit vorlag. Diese bisher größte Untersuchung von T-Zell-Markern an H-/RS-Zellen, erstmalig auch an NLP-HL, ergab eine bis zu 6-fach höhere Frequenz in der NLP-HL bei häufigerer B-Zell-Marker-Expression. Die in der Literatur beschriebene Rangordnung der Expressionshäufigkeit von Oberflächenantigenen im c-HL (CD2 > CD4 > CD3 > CD5 > CD8) wurde bestätigt und wich nur in den Markern CD3 und CD5 ab: Perforin >> CD4 > CD5 > CD3 > CD8 > GranzymB > TIA-1 > CD7. Tzankov et al. [45] fanden in ihrer Untersuchung mit 259 c-HL-Fällen eine Häufigkeit von 5% T-Zell-Marker-Expression. Die vorliegende Arbeit mit 1147 untersuchten c-HL-Fällen kam zum Ergebnis einer deutlich höheren T-Zell-Marker-Expressionshäufigkeit von 20,1%. Der pathophysiologische Mechanismus der T-Zell-Marker-Expression ist bis heute noch unklar, könnte aber als eine alternative Signalkaskade zur Aufrechterhaltung des Zellzyklus unter geändertem Zellmilieu gedeutet werden. Ein weiterer Fokus dieser Arbeit betraf die Gruppe der Studienteilnehmer 60 Jahre und älter, um Hinweisen auf das „age-related“ EBV-associated Lymphom und der Rolle der Mikrosatelliten-Instabilität nachzugehen. So fand sich eine signifikante Häufung von Markern für eine EBV-Infektion (EBER, LMP1) in der Gruppe der über 60-Jährigen. Die Expression von DNA-Reparaturenzymen, deren Ausbleiben auf Mikrosatelliten-Instabilität gedeutet hätte, unterschied sich zwischen jüngeren und älteren Studienteilnehmern nicht.
Hirntumoren werden nach histologischen und molekulargenetischen Gesichtspunkten unterteilt. Neben dem Krankheitsverlauf unterscheiden sich LGA auch genetisch von GBM. Wie die mRNA der Proteine ATF5, KiSS1, RPS27, BRMS1 und TTK in LGA exprimiert ist bzw. sich im Verlauf verändert, war in dieser Form noch nicht in einem Patientenpanel untersucht worden. Ziel dieser Arbeit war es, die mRNA-Expressionsraten zu bestimmen sowie Korrelationen und Kointegrationen mit klinischen Daten zu analysieren. Außerdem wurden Besonderheiten der Verteilung innerhalb des Patientenpanels beschrieben. Quantitative-PCR-Analysen wurden durchgeführt. Die Expressionswerte wurden auf die Expression des Haushaltsgens GAPDH normalisiert. Die resultierenden ∆CTm - bzw. ∆∆CT-Werte sowie die klinischen Patientendaten waren Basis für Kointegrations-, Korrelations-, Expressions-, Ähnlichkeits- und Verlaufsanalysen. KiSS1 schien generell kaum oder nicht in der Vielzahl der Tumoren exprimiert zu sein, Aussagen zur klinischen Korrelation erwiesen sich als schwierig. Für RPS27 konnten tendenziell niedrigere Werte in den Verlaufstumoren im Vergleich zu den LGA gefunden werden. Auch für BRMS1 war in der Mehrzahl der Fälle die mRNA der Vorläufertumoren in Relation zu den Rezidiven stärker exprimiert. ATF5 korrelierte nach Kendall RE und PFS (-0,324; p=0,077) in der Gruppe der IDH1-mutierten LGA, für TTK nach Kendall OS und RE (-0,444; p=0,097) im Gesamtpanel und nach Pearson auch RE und PFS (-0,43; p=0,096) in der Gruppe der IDH1-mutierten LGA.
Interleukin-5 ist ein Th2-Cytokin, das eosinophile Granulozyten aktiviert und B-Zellen zur Produktion von IgE stimuliert. Bei der Entstehung von allergischen (wie z.B. Asthma) und atopischen Reaktionen spielt die erhöhte Ausschüttung von IL-5 eine wichtige Rolle. Der Interleukin-5-Promoter weist unter anderem Bindestellen für NF-AT-Faktoren und GATA-3 auf. NF-ATc ist ein Mitglied der NF-AT („Nuclear Factor of Activated T-cells“)-Transkriptionsfaktoren, die an den verschiedensten immunologischen Funktionen beteiligt sind, vor allem aber an der Steuerung der Cytokingene. GATA-3 ist ein wichtiger Th2-spezifischer Zinkfinger-Transkriptionsfaktor aus der Familie der GATA-Faktoren, die an eine gemeinsame WGATAR-Sequenz der DNA binden. Molkentin et al. zeigten 1998, daß NF-AT3 und GATA-4 in Herzmuskelzellen physikalisch interagieren und daß ihre funktionelle Kooperation bei der Aktivierung verschiedener Promotoren letztendlich zur Entwicklung einer Herzhypertrophie beiträgt. In dieser Arbeit sollte untersucht werden, ob eine ähnliche physikalische Interaktion zwischen NF-ATc und GATA-3 in T-Zellen stattfindet. Zu diesem Zweck wurden im ersten Teil der Arbeit Coimmunopräzipitationen durchgeführt. Dabei konnte in mit NF-ATc und GATA-3 cotransfizierten 293T Zellen eine spezifische in vivo Interaktion der beiden Transkriptionsfaktoren nachgewiesen werden. Im zweiten Teil der Arbeit sollten mittels GST-„pulldown“-Experimenten die für die Interaktion wichtigen Proteindomänen von NF-ATc und GATA-3 bestimmt werden. Im ersten Schritt wurden die dafür benötigten Plasmide konstruiert. Im zweiten Schritt erfolgte die bakterielle Expression und nachfolgende Aufreinigung der GST-Fusionsproteine. Mit GST wurde jeweils eine N- und C-terminale Hälfte von NF-ATc und GATA-3 fusioniert. Mit diesen rekombinanten Proteinen wurden die „pulldown“-Experimente durchgeführt. Dabei konnte eine Interaktion des C-terminalen Anteils (enthält den zweiten Zinkfinger) von GATA-3 mit NF-ATc detektiert werden. Nachfolgende Ergebnisse deuteten auf eine Interaktion des C-terminalen Anteils (enthält die „Rel-Similarity-Domain“) von NF-ATc mit GATA-3 hin. Analog zeigten Molkentin et al., daß die RSD von NF-AT3 mit dem C-terminalen Zinkfinger von GATA-4 in Herzmuskelzellen interagiert. Die Interaktion von NF-ATc und GATA-3 scheint nicht nur physikalisch zu existieren, sondern auch funktionell von Bedeutung zu sein. In Luciferase-Reporteressays, die in unserem Labor durchgeführt wurden, zeigte sich bei Cotransfektion von NF-ATc und GATA-3 im Vergleich zu Einzeltranfektionen eine drastische Aktivitätssteigerung des IL-5 Promoters. Diese Ergebnisse weisen – wiederum analog zu den Vorgänge im Herzen - auf eine funktionelle Kooperation der beiden Transkriptionsfaktoren bei der Steuerung des IL-5 Promoters hin.
Das Follikuläre Lymphom (FL) ist nach dem Diffusen großzelligen B-Zell-Lymphom (DLBCL) das häufigste Non-Hodgkin-Lymphom in der westlichen Welt. Die aktuelle WHO-Klassifikation für Tumoren der Hämatopoetischen und Lymphoiden Gewebe aus dem Jahr 2008 unterteilt dieses maligne Lymphom nach Histologie und Wachstumsmuster in vier Gruppen, FL 1 bis FL 3A und FL 3B. Obwohl die FL 1 und FL 2 zu den indolenten Tumoren gezählt werden, und FL 3A und FL 3B tendenziell eher als aggressiv gelten, so wurde in einigen Studienarbeiten gezeigt, dass das FL 3A aufgrund seiner immunhistologischen und genetischen Charakteristika, insbesondere dem Vorhandensein der BCL2/IGH t(14;18)(q32;q21) Translokation, eher den low-grade-Lymphomen (FL 1 und FL 2) nahe steht, während das FL 3B durchaus Eigenschaften des DLBCL, wie das Fehlen einer BCL2/IGH-Translokation und das vermehrte Auftreten von Aberrationen des BCL6-Gens, zeigt. In verschiedenen Arbeiten wurde des Weiteren eine Einteilung in reine FL 3B und FL 3B mit Anteilen eines DLBCL (+ DLBCL) vorgenommen, da sich auch diese beiden Subgruppen durch unterschiedliche Proteinexpression und genetische Eigenschaften auszeichnen würden. Laut den bislang in der Literatur vorliegenden (spärlichen) Daten zeigen FL 3A und FL 3B unterschiedliche Antigen-Profile und offenbar auch unterschiedliche (primäre) genetische Veränderungen, wobei gerade für das FL 3B nur wenige Daten vorliegen. Während Grad 3A-Tumoren einige Ähnlichkeiten zu den FL 1 und 2 zeigen, scheint das FL 3B im Immunphänotyp wie in der Genetik eher dem DLBCL zu ähneln. Allerdings lässt sich bei kritischer Durchsicht der Literatur erkennen, dass die meisten Fälle eines FL Grad 3 entweder gar nicht den Graden 3A oder 3B zugeordnet, beziehungsweise in diese unterschieden wurden, oder häufig bereits einen zusätzlichen diffusen Wachstumstyp aufweisen, nach den Regeln der WHO-Klassifikation für Tumoren der Hämatopoetischen und Lymphatischen Gewebe (2008) also als DLBCL mit einem zusätzlichen follikulären Wachstumsanteil klassifiziert würden. Somit sind die Daten insbesondere über die rein follikulär wachsenden FL 3B äußerst spärlich. Die vorliegende Arbeit beschäftigt sich mit der immunhistochemischen und genetischen Charakterisierung der FL 3B. Von besonderem Interesse war die Bestimmung der Häufigkeit der BCL2/IGH t(14;18)(q32;q21), BCL6/IGH t(3;14)(q27;q32) und MYC/IGH t(8;14)(q24;q32) Translokationen in den verschiedenen Typen der FL. Weiterhin sollte der Frage nachgegangen werden, ob die Anwendung der Tissue Microarray (TMA)-Technik und der Fluoreszenz in situ Hybridisierung (FISH) an TMAs robuste Daten zu dieser Fragestellung liefern kann. In einem ersten Schritt wurden vorhandene TMAs von FL, DLBCL und MALT-Lymphomen mit break-apart-Sonden für BCL2, BCL6, MYC und IGH hybridisiert, und die gewonnenen Ergebnisse mit Daten der konventionellen Zytogenetik abgeglichen. Hierdurch sollte nachgewiesen werden, dass die FISH in Kombination mit der TMA-Technik eine valide Testmethode zur Aufdeckung der gesuchten chromosomalen Aberrationen darstellt, die in Sensitivität und Spezifität der klassischen Zytogenetik nicht nachsteht. In einem zweiten Schritt wurden FL aus dem Archiv des Pathologischen Instituts der Universität Würzburg anhand der aktuellen WHO-Kriterien in die Grade 1, 2, 3A und 3B eingeteilt (und reklassifiziert). Diese Tumoren wurden im TMA und im Vollschnitt durch immunhistochemische Färbungen auf ihre Protein-Expression und mittels FISH auf ihre genetischen Eigenschaften untersucht und charakterisiert.
p53 protects us from cancer by transcriptionally regulating tumor suppressive programs designed to either prevent the development or clonal expansion of malignant cells. How p53 selects target genes in the genome in a context-and tissue-specific manner remains largely obscure. There is growing evidence that the ability of p53 to bind DNA in a cooperative manner prominently influences target gene selection with activation of the apoptosis program being completely dependent on DNA binding cooperativity. Here, we used ChIP-seq to comprehensively profile the cistrome of p53 mutants with reduced or increased cooperativity. The analysis highlighted a particular relevance of cooperativity for extending the p53 cistrome to non-canonical binding sequences characterized by deletions, spacer insertions and base mismatches. Furthermore, it revealed a striking functional separation of the cistrome on the basis of cooperativity; with low cooperativity genes being significantly enriched for cell cycle and high cooperativity genes for apoptotic functions. Importantly, expression of high but not low cooperativity genes was correlated with superior survival in breast cancer patients. Interestingly, in contrast to most p53-activated genes, p53-repressed genes did not commonly contain p53 binding elements. Nevertheless, both the degree of gene activation and repression were cooperativity-dependent, suggesting that p53-mediated gene repression is largely indirect and mediated by cooperativity-dependently transactivated gene products such as CDKN1A, E2F7 and non-coding RNAs. Since both activation of apoptosis genes with non-canonical response elements and repression of pro-survival genes are crucial for p53's apoptotic activity, the cistrome analysis comprehensively explains why p53-induced apoptosis, but not cell cycle arrest, strongly depends on the intermolecular cooperation of p53 molecules as a possible safeguard mechanism protecting from accidental cell killing.
SphK1 is known to play a role in tumor progression, resistance to radiochemotherapy, and migration patterns. As the overall survival rates of squamous cell carcinoma of the head and neck (HNSCC) remain poor due to limitations in surgery and irradiation and chemotherapy resistance, SphK1 is an important enzyme to investigate. The purpose of this study was to elucidate the impact of SphK1 on irradiation efficacy of HNSCC in-vitro with emphasis on EGFR signaling. By immunhistochemical staining we found a positive correlation between EGFR and SphK1 expression in patient specimens. In colony formation assays irradiation sensitive cell lines showed a poor response to cetuximab, an EGFR inhibitor, and SKI-II, a SphK1 inhibitor, and vice versa. In irradiation sensitive cells an enhanced reduction of cell migration and survival was found upon simultaneous targeting of EGFR and SphK1. In the present study, we elucidated a linkage between the two signaling pathways with regard to the efficacy of cetuximab treatment and the impact on the migration behavior of tumor cells. We investigated the biological impact of inhibiting these pathways and examined the biochemical implications after different treatments. An understanding of the processes involved could help to improve the treatment of patients with HNSCC.
Der Transkriptionsfaktor C/EBPβ besitzt sehr vielgestaltige Funktionen und ist an Wachstums-und Differenzierungsvorgängen verschiedener Gewebe beteiligt. So fördert es in T-Lymphozyten über Transaktivierung des Il4-Promotors und Repression der TH1-Zytokine IL-2 und IFN-γ die Bildung eines TH2-Phänotyps [Berberich-Siebelt et al. 2000]. Durch Herabregulation von c-Myc bewirkt es einen Zellzyklusarrest in G1 und vermehrte Differenzierung der Zellen auch über eine reziproke Steigerung von Differenzierungsfaktoren wie Mad4 [Berberich-Siebelt et al. 2006]. In einer den G1-Arrest nachweisenden Zellzyklusanalyse von mit C/EBPβ transduzierten EL-4 Zellen zeigte sich daneben ein kleiner Sub-G1-Peak, der auf eine apoptotische Zellpopulation hinweist [Berberich-Siebelt et al. 2006]. Aufgabe dieser Arbeit war es, den möglichen Zusammenhang zwischen der Aktivierung von C/EBPβ und Auslösung von Apoptose in EL-4 Zellen hinsichtlich seiner Spezifität und dabei favorisierter Signalwege zu untersuchen.
Gegenstand der Untersuchungen waren mit dem C/EBPβ-ERTM-Konstrukt alleine und in Kombination mit dominant-negativen Mutanten der Caspase-3 und der Caspase-9 transduzierte EL-4 Kulturzellen. Durch Einbringen der Caspasemutanten sollte eine kompetitive Hemmung der entsprechenden endogenen Caspasen bewirkt werden. Methodisch erfolgten Apoptosenachweise mittels durchflusszytometrischer Analysen von mit Annexin V-PE und 7-Amino-Actinomycin (7-AAD) gefärbten EL-4 Zellen sowie die Detektion von gespaltener PARP (Poly-ADP-Ribose-Polymerase), einem Substrat der Caspase-3 im Western Blot. Des Weiteren erfolgten mittels Ribonuklease-Protektionsanalysen Untersuchungen der RNA-Expression von Zytokinen, Caspasen und von Mitgliedern der Myc- und der Bcl-2-Proteinfamilien, um das Verhalten der Zellen unter Hemmung von Apoptosewegen bzw. Caspasen bei Aktivierung von C/EBPβ näher betrachten zu können.
In Annexin V-PE- und 7-AAD-Färbungen sowie durch Nachweis der spezifischen Spaltung von PARP konnte gezeigt werden, dass C/EBPβ Zelluntergang und Apoptose fördert. Diese war durch den Pancaspaseinhibitor Z-VAD fmk hemmbar, was, wie auch die PARP-Spaltung, auf einen caspaseabhängigen Signalweg hinweist. Hemmung der Caspase-3 durch Transduktion der Zellen mit einer Caspase-3-Mutante besaß kaum Einfluss auf die durch C/EBPβ veränderte Zytokinexpression und die Repression von c-Myc, doch erschien eine vermehrte Hochregulation des Differenzierungsfaktors Mad4, der endogenen Caspase3- und der Caspase11-RNA. Die Steigerung von Caspase-3 unter Aktivierung von C/EBPβ fand sich auch auf Proteinebene. Allerdings konnte eine Hemmung der Caspase-3 bei den untersuchten EL-4 Zellen die durch C/EBPβ vermittelte Apoptose nicht verhindern, was auf andere Apoptosewege oder kompensatorischer Effekte verwies. Durch Beeinflussung des intrinsischen Signalweges mit Hemmung der Caspase-9 zeigten sich ebenfalls kaum Auswirkungen auf die Zytokinexpression der untersuchten Zellen. Hier fanden sich Hochregulationen sowohl der pro- als auch antiapoptotischen Mitglieder der Bcl-2-Familie. Funktionell konnte auch eine Hemmung von Caspase-9 die Zellen nicht vor der Apoptose durch Aktivierung von C/EBPβ bewahren.
So konnte hier gezeigt werden, dass Aktivierung von C/EBPβ in den untersuchten EL-4 Zellen Apoptose fördern kann, dies über eine Aktivierung der Caspasekaskade zu geschehen scheint und mit einer Steigerung endogener Caspase-3-Expression einhergeht. Aus den Untersuchungen dieser Arbeit konnte eine Favorisierung eines bestimmten Apoptosesignalweges nicht abgeleitet werden, da eine Hemmung des intrinsischen Weges die Zellen nicht vor dem Zelltod schützen konnte. Insgesamt lässt sich aber, obwohl nicht alle Details geklärt werden konnten, festhalten, dass C/EBPβLAP in T-Zellen neben Proliferationshemmung und Differenzierungsinduktion auch für Caspase vermittelten Zelltod verantwortlich ist.
Der humane, monoklonale IgG Antikörper BARB-4 konnte mit Hilfe der Hybridomatechnologie aus einem an Magenkarzinom erkrankten Patienten isoliert werden. BARB-4 stellt aufgrund seiner Keimbahnkodierung einen Bestandteil der innaten Immunität dar und ist eines der wenigen tumorspezifischen IgG Immunglobuline, die diesem Teil des Immunsystems zugeordnet werden können. Innerhalb dieser Arbeit konnte die Zielstruktur des Antikörpers identifiziert und näher charakterisiert werden. Das BARB-4 Antigen wurde hierbei über ein affinitätschromatographisches Verfahren aus Tumorzellmembranextrakten aufgereinigt und anschließend mittels MALDI-MS über die Peptidmassen-Fingerprint Methode analysiert. Das dabei isolierte Protein konnte eindeutig als humanes TAF15 identifiziert werden. Diese auf der Zellmembran von Tumoren exprimierte TAF15 Variante besitzt ein Molekulargewicht von etwa 78 kDa. Sie kommt im Gegensatz zum Wildtyp exklusiv in Tumorzellen vor und konnte nicht in Normalgewebe nachgewiesen werden. Immunhistochemische Untersuchungen mit BARB-4 und Anti-TAF15 Antikörper auf Tumor- und Normalgewebe deuteten dabei auf eine Koexistenz von Wildtyp und tumorspezifischer TAF15-Variante in malignem Gewebe hin und legten somit eine tumorspezifische Modifikation des TAF15BARB-4 nahe. Ein Carbohydrat-Epitop, wie es sehr häufig bei den natürlichen IgM Antikörpern vorkommt, konnte hier jedoch ausgeschlossen werden. In funktionellen Analysen konnte gezeigt werden, dass die Bindung des BARB-4 Antikörpers auf Tumorzellen einen Einfluss auf diverse zelluläre Prozesse ausübt. Durch die Bindung hemmte der Antikörper das Zellwachstum von Tumorzellen und induzierte deren Apoptose. Weitere interessante Eigenschaften des BARB-4, die bei Tumorzellen beobachtet werden konnten, sind vor allem für metastasierende Zellen von Bedeutung. Nach erfolgter Antikörperinkubation konnte bei Tumorzellen eine Inhibierung der Zelladhäsion und der Zellbeweglichkeit nachgewiesen werden. Diese beiden zellulären Prozesse sind wichtig für sich im Körper ausbreitende, maligne Zellen. In allen durchgeführten Analysen handelte es sich um vom Antikörper direkt vermittelte Effekte. Weitere Untersuchungen wurden durchgeführt, um das Bindungsverhalten des Antikörpers genauer charakterisieren zu können. Immunfluoreszenzanalysen zeigten dabei, dass der Antikörper BARB-4 nach der Bindung an die Tumorzellmembran internalisiert wird. Die Erforschung des BARB-4 Antikörpers und seiner Zielstruktur TAF15BARB-4 auf Krebszellen ermöglicht sowohl neue Einblicke in die Funktionsweise der innaten Immunität als auch neue Optionen für die zielgerichtete Tumortherapie. Die Identifizierung einer extrazellulären, tumorspezifischen TAF15 Variante bietet eine neue Möglichkeit für Diagnostik- und Therapieansätze. Durch die exklusive Expression auf Tumorzellen ermöglicht diese TAF15-Variante gezielt maligne Zellen zu attackieren ohne dabei gesunde Zellen zu beeinflussen. Durch das Vorkommen des TAF15BARB-4 in den verschiedensten Tumorentitäten könnte diese Zielstruktur für die Therapie vieler unterschiedlicher, maligner Erkrankungen genutzt werden. Aufgrund seiner funktionellen Eigenschaften, wie der Hemmung der Tumorzellmotilität und Tumorzelladhäsion, könnte der BARB-4 Antikörper besonders für die Prävention einer Metastasierung von Bedeutung sein.
Adrenocortical tumors consist of benign adenomas and highly malignant carcinomas with a still incompletely understood pathogenesis. A total of 46 adrenocortical tumors (24 adenomas and 22 carcinomas) were investigated aiming to identify novel genes involved in adrenocortical tumorigenesis. High-resolution single nucleotide polymorphism arrays (Affymetrix) were used to detect copy number alterations (CNAs) and copy neutral losses of heterozygosity (cnLOH). Genomic clustering showed good separation between adenomas and carcinomas, with best partition including only chromosome 5, which was highly amplified in 17/22 malignant tumors. The malignant tumors had more relevant genomic aberrations than benign tumors, such as a higher median number of recurrent CNA (2631 vs 94), CNAs >100 Kb (62.5 vs 7) and CN losses (72.5 vs 5.5), and a higher percentage of samples with cnLOH (91% vs 29%). Within the carcinoma cohort, a precise genetic pattern (i.e. large gains at chr 5, 7, 12, and 19, and losses at chr 1, 2, 13, 17, and 22) was associated with a better prognosis (overall survival: 72.2 vs 35.4 months, P=0.063). Interestingly, >70% of gains frequent in beningn were also present in malignant tumors. Notch signaling was the most frequently involved pathway in both tumor entities. Finally, a CN gain at imprinted “IGF2” locus chr 11p15.5 appeared to be an early alteration in a multi-step tumor progression, followed by the loss of one or two alleles, associated with increased IGF2 expression, only in carcinomas. Our study serves as database for the identification of genes and pathways, such as Notch signaling, which could be involved in the pathogenesis of adrenocortical tumors. Using these data, we postulate an adenoma-carcinoma sequence for these tumors.
Adrenocortical carcinoma (ACC) is a rare endocrine malignancy and treatment of advanced disease is challenging. Clinical trials with multi-tyrosine kinase inhibitors in the past have yielded disappointing results. Here, we investigated fibroblast growth factor (FGF) receptors and their pathways in adrenocortical tumors as potential treatment targets. We performed real-time RT-PCR of 93 FGF pathway related genes in a cohort of 39 fresh frozen benign and malignant adrenocortical, 9 non-adrenal tissues and 4 cell lines. The expression of FGF receptors was validated in 166 formalin-fixed paraffin embedded (FFPE) tissues using RNA in situ hybridization (RNAscope) and correlated with clinical data. In malignant compared to benign adrenal tumors, we found significant differences in the expression of 16/94 FGF receptor pathway related genes. Genes involved in tissue differentiation and metastatic spread through epithelial to mesechymal transition were most strongly altered. The therapeutically targetable FGF receptors 1 and 4 were upregulated 4.6- and 6-fold, respectively, in malignant compared to benign adrenocortical tumors, which was confirmed by RNAscope in FFPE samples. High expression of FGFR1 and 4 was significantly associated with worse patient prognosis in univariate analysis. After multivariate adjustment for the known prognostic factors Ki-67 and ENSAT tumor stage, FGFR1 remained significantly associated with recurrence-free survival (HR=6.10, 95%CI: 1.78 – 20.86, p=0.004) and FGFR4 with overall survival (HR=3.23, 95%CI: 1.52 – 6.88, p=0.002). Collectively, our study supports a role of FGF pathways in malignant adrenocortical tumors. Quantification of FGF receptors may enable a stratification of ACC for the use of FGFR inhibitors in future clinical trials.
A clinically relevant proportion of adrenocortical carcinoma (ACC) cases shows a tendency to metastatic spread. The objective was to determine whether the epithelial to mesenchymal transition (EMT), a mechanism associated with metastasizing in several epithelial cancers, might play a crucial role in ACC. 138 ACC, 29 adrenocortical adenomas (ACA), three normal adrenal glands (NAG), and control tissue samples were assessed for the expression of epithelial (E-cadherin and EpCAM) and mesenchymal (N-cadherin, SLUG and SNAIL) markers by immunohistochemistry. Using real-time RT-PCR we quantified the alternative isoform splicing of FGFR 2 and 3, another known indicator of EMT. We also assessed the impact of these markers on clinical outcome. Results show that both normal and neoplastic adrenocortical tissues lacked expression of epithelial markers but strongly expressed mesenchymal markers N-cadherin and SLUG. FGFR isoform splicing confirmed higher similarity of adrenocortical tissues to mesenchymal compared to epithelial tissues. In ACC, higher SLUG expression was associated with clinical markers indicating aggressiveness, while N-cadherin expression inversely associated with these markers. In conclusion, we could not find any indication of EMT as all adrenocortical tissues lacked expression of epithelial markers and exhibited closer similarity to mesenchymal tissues. However, while N-cadherin might play a positive role in tissue structure upkeep, SLUG seems to be associated with a more aggressive phenotype.
The cellular microenvironment in follicular lymphoma is of biological and clinical importance. Studies on the clinical significance of non-malignant cell populations have generated conflicting results, which may partly be influenced by poor reproducibility in immunohistochemical marker quantification. In this study, the reproducibility of manual scoring and automated microscopy based on a tissue microarray of 25 follicular lymphomas as compared to flow cytometry is evaluated. The agreement between manual scoring and flow cytometry was moderate for CD3, low for CD4, and moderate to high for CD8, with some laboratories scoring closer to the flow cytometry results. Agreement in manual quantification across the 7 laboratories was low to moderate for CD3, CD4, CD8 and FOXP3 frequencies, moderate for CD21, low for MIB1 and CD68, and high for CD10. Manual scoring of the architectural distribution resulted in moderate agreement for CD3, CD4 and CD8, and low agreement for FOXP3 and CD68. Comparing manual scoring to automated microscopy demonstrated that manual scoring increased the variability in the low and high frequency interval with some laboratories showing a better agreement with automated scores. Manual scoring reliably identified rare architectural patterns of T-cell infiltrates. Automated microscopy analyses for T-cell markers by two different instruments were highly reproducible and provided acceptable agreement with flow cytometry. These validation results provide explanations for the heterogeneous findings on the prognostic value of the microenvironment in follicular lymphoma. We recommend a more objective measurement, such as computer-assisted scoring, in future studies of the prognostic impact of microenvironment in follicular lymphoma patients.
Der menschliche Organismus ist zeitlebens von malignen Neoplasien bedroht, die durch lokales oder metastasiertes Wachstum lebensnotwendige Funktionen des Körpers beeinträchtigen können. Als wichtigstes Werkzeug zur Abwehr dieser Neoplasien wurde in den letzten Jahrzehnten die natürliche Immunität aufgedeckt. Besonders die Antikörper der innaten Immunität spielen eine entscheidende Rolle. BARB-4 ist ein humaner, tumorspezifischer Antikörper und Teil dieser natürlichen Immunität. Er wurde mit Hilfe der Hybridomatechnologie aus einem Patienten mit Siegelringkarzinom des Magens isoliert, und ist einer der wenigen Vertreter innater humaner IgG Antikörper. Diese Arbeit gibt einen ersten Überblick über die Bindungsspezifität und die funktionellen Eigenschaften des BARB-4-Antikörpers. In den immunhistochemischen Färbungen konnte die Tumorspezifität des Antikörpers nachgewiesen werden. Bei dem zugehörigen Antigen handelt es sich um eine Variante des TAF15, einem Protein der FET-Familie, die intrazelluläre Aufgaben bei Transkriptionsvorgängen haben, bei denen zudem aber auch eine Beteiligung an Adhäsions- und Migrationsvorgängen vermutet wird. Diese Variante ist bei malignen Zellen an der Oberfläche lokalisiert, was die Ergebnisse der Durchflußzytometrie belegen. Durch konfokale Mikroskopie mit Fluoreszenz-markiertem BARB-4 konnte diese Oberflächenbindung an Tumorzellen bestätigt werden. Im weiteren zeitlichen Verlauf konzentrierte sich der Antikörper im Zellinneren. Die Präsenz des Antikörpers führte bei Versuchen mit Tumorzellen zu einer bemerkenswerten Hemmung der Adhäsions- und Migrationsfähigkeit der Zellen. Beide stellen Schlüsseleigenschaften für die Metastasierung von Tumorzellen dar. Diese Eigenschaften könnten BARB-4 für einen möglichen, therapeutischen Einsatz zur Prävention von Tumormetastasen qualifizieren.
Die Synovialmembran zeigt bei Osteoarthrose, einer primär degenerativen Erkrankung, ein sekundäres entzündliches Geschehen. Histopathologisch existiert das entzündliche Infiltrat in zwei unterschiedlichen Mustern, die jedoch eine sekundäre, durch Knorpelalteration bedingte Synovialitis gemeinsam haben: (1) Detritus Synovialitis und (2) lympho-plasmazelluläre Synovialitis, beide mit leicht ausgeprägter entzündlicher Infiltration durch Lymphozyten und Plasmazellen. Ziel dieser Studie war es, ein genaueres Verständnis von der B-Zell-Aktivierung in der Synovialmembran der OA zu eruieren. Denn es ist immer noch ungeklärt, ob die B-Lymphozyten im synovialen Infiltrat Antigen-aktiviert sind, als solche möglicherweise schon einwandern, oder ob sie auch lokal expandieren (im Sinne einer antigenabhängigen Affinitätsmaturation), wie bei der RA. Bei dieser ist inzwischen eine lokale Affinitätsmaturation in synovialen Keimzentren bewiesen. Dazu wurden IgVH-Gene der synovialen B-Lymphozyten analysiert und immunhistochemische Färbungen durchgeführt, da bis dato noch wenige morphologische, als auch molekularbiologische Daten über die synovialen B-Zellen bzw. Plasmazellen in der OA vorliegen. Ein Fokus wurde auf die Antigen-Aktivierung der synovialen B-Lymphozyten (charakterisiert durch hohe R/S-Ratios) gerichtet. Um der Herkunft des Entzündungsgeschehens nachzugehen, wurde die Histologie verschiedener Differenzierungsstufen von B-Lymphozyten im entzündlichen Infiltrat von nicht-follikulären und follikelähnlichen B-Zell-Ansammlugen analysiert. Durch CD27/CD20– und CD27/Syndekan-Doppelfärbungen konnte die bevorzugte Lokalisation von Memory-und Plasma-Zellen identifiziert werden. Diese gab nähere Hinweise auf den Ort der Antigen-Aktivierung bei Osteoarthrose. Die Resultate der hohen R/S-Ratios (17/24 Klonen), gerade in den CDR-Regionen, stehen im Einklang mit der gängigen Meinung, dass die CDR-Regionen mehr mutieren als die FR-Regionen, da in diesen eher ein konstantes Antikörpermerkmal erhalten wird, wohingegen die CDR´s auf eine Affinitätsanpassung (durch höhere Mutationsraten) ausgerichtet sind. Die Mutationsrate weist auf eine stattgefundene Antigenaktivierung dieser B-Lymphozyten hin. Aus den histologischen Schnittbildern mit den oben genannten Doppelfärbungen war v.a. perivaskulär ein Vorkommen von vielen CD27+/CD20+ Memory-B-Zellen (PMZ/IaM), sowie Plasmazellen zu erkennen, dahingegen relativ betrachtet wenige CD27-CD20+ B-Lymphozyten. Dies deutet darauf hin, dass bei der OA vermehrt bereits aktivierte Memory-Zellen ins Synovialgewebe einwandern. Allerdings zeigte sich eine unerwartete Anhäufung von Follikelähnlichen Formationen ohne Keimzentrumscharakter, die Anlaß zu weiteren Überlegungen geben. Die Diskussion findet im Rahmen des generellen Pathogenese-Konzepts der OA statt, mit besonderem Augenmerk auf die unterschiedlichen Entzündungstypen und -muster, sowie die dominierenden Zelltypen in der osteoarthrotischen Synovialmembran. Da in dem Entzündungsinfiltrat der osteoarthrotischen Synovialmembran keine Keimzentren zu finden sind, nur Follikelähnliche Formationen, muss von einer Antigen-Aktivierung und Affinitätsreifung außerhalb des Gelenkes ausgegangen werden. Bei nahezu allen morphologischen Betrachtungen der osteoarthrotischen Synovia fiel eine deutlich hohe Anzahl von CD27+/CD20+ Memory-B-Lymphozyten perivaskulär auf. Daher kann davon ausgegangen werden, dass wir es beim arthrotischen Entzündungsinfiltrat mit einer Sekundär-Antwort zu tun haben, die auf Antigene reagiert, an die sich das Immunsystem „erinnert“ und bereits aktivierte B-Zellen einwandern. Zusammenfassend legt die Datenlage nahe, dass man offensichtlich von einer immunologischen Bekanntheit es körpereigenen Immunsystems mit freigesetzten Antigenen in der OA spechen kann. Hierdurch wurde ein Beitrag zum immunpathogenetischen Verständnis der sekundären Begleitsynovialitis dahingehend gefunden, dass die geringer ausgeprägte Entzündung in der Arthrose -im Gegensatz zur RA- ein Ausdruck der Reaktivität auf „dem Immunsystem in Erinnerung gebliebener“ Antigene ist. Der Vergleich der potentiellen Antigene bei den Krankheitsbildern RA und OA weist einige Gemeinsamkeiten auf, wodurch sich die Frage einer genetischen Prädisposition für das Ausmaß des Entzündungsgrades in den Gelenken ergibt.
The transcription factor NFATc1 has been shown to regulate the activation and differentiation of T-cells and B-cells, of DCs and megakaryocytes. Dysregulation of NFAT signaling was shown to be associated with the generation of autoimmune diseases, malignant transformation and the development of cancer [71]. The primary goal of this work was to gain insights on Nfatc1 induction and regulation in lymphocytes and to find new direct NFATc1 target genes. Three new BAC -transgenic reporter mouse strains (tgNfatc1/Egfp, tgNfatc1/DE1 and tgNfatc1/DE2) were applied to analyze Nfatc1 induction and regulation in primary murine B- and T-cells. As a result, we were able to show the persistent requirement of immunoreceptor-signaling for constant Nfatc1 induction, particularly, for NFATc1/αA expression. Furthermore, we showed that NF-κB inducing agents, such as LPS, CpG or CD40 receptor engagement, in combination with primary receptor-signals, positively contributed to Nfact1 induction in B-cells [137]. We sought to establish a new system which could help to identify direct NFATc1 target genes by means of ChIP and NGS in genom-wide approaches. We were able to successfully generate a new BAC-transgene encoding a biotinylatable short isoform of NFATc1, which is currently injected into mice oocyte at the TFM in Mainz. In addition, in vivo biotinylatable NFATc1–isoforms were cloned and stably expressed in the murine B-cell lymphoma line WEHI-231. The successful use of these cells stably overexpressing either the short NFATc1/αA or the long NFATc1/βC isoform along with the bacterial BirA biotin ligase was confirmed by intracellular stainings, FACS analysis, confocal microscopy and protein IP. By NGS, we detected 2185 genes which are specifically controlled by NFATc1/αA, and 1306 genes which are exclusively controlled by NFATc1/βC. This shows that the Nfatc1 locus encodes “two genes” which exhibit alternate, in part opposite functions. Studies on the induction of apoptosis and cell-death revealed opposed roles for the highly inducible short isoform NFATc1/αA and the constantly expressed long isoform NFATc1/βC. These findings were confirmed by whole transcriptome-sequencing performed with cells overexpressing NFATc1/αA and NFATc1/βC. Several thousand genes were found to be significantly altered in their expression profile, preferentially genes involved in apoptosis and PCD for NFATc1/βC or genes involved in transcriptional regulation and cell-cycle processes for NFATc1/αA. In addition we were able to perform ChIP-seq for NFATc1/αA and NFATc1/βC in an ab-independent approach. We found potential new target-sites, but further studies will have to address this ambitious goal in the future. In individual ChIP assays, we showed direct binding of NFATc1/αA and NFATc1/βC to the Prdm1 and Aicda promoter regions which are individually controlled by the NFATc1 isoforms.
The transcription factor FOXP1 is implicated in the pathogenesis of B-cell lymphomas through chromosomal translocations involving either immunoglobulin heavy chain (IGH) locus or non-IG sequences. The former translocation, t(3; 14)(p13; q32), results in dysregulated expression of FOXP1 juxtaposed with strong regulatory elements of IGH. Thus far, molecular consequences of rare non-IG aberrations of FOXP1 remain undetermined. Here, using molecular cytogenetics and molecular biology studies, we comprehensively analyzed four lymphoma cases with non-IG rearrangements of FOXP1 and compared these with cases harboring t(3; 14)(p13; q32)/IGH-FOXP1 and FOXP1-expressing lymphomas with no apparent structural aberrations of the gene. Our study revealed that non-IG rearrangements of FOXP1 are usually acquired during clinical course of various lymphoma subtypes, including diffuse large B cell lymphoma, marginal zone lymphoma and chronic lymphocytic leukemia, and correlate with a poor prognosis. Importantly, these aberrations constantly target the coding region of FOXP1, promiscuously fusing with coding and non-coding gene sequences at various reciprocal breakpoints (2q36, 10q24 and 3q11). The non-IG rearrangements of FOXP1, however, do not generate functional chimeric genes but commonly disrupt the full-length FOXP1 transcript leading to an aberrant expression of N-truncated FOXP1 isoforms (FOXP1NT), as shown by QRT-PCR and Western blot analysis. In contrast, t(3; 14)(p13; q32)/IGH-FOXP1 affects the 59 untranslated region of FOXP1 and results in overexpress the full-length FOXP1 protein (FOXP1FL). RNA-sequencing of a few lymphoma cases expressing FOXP1NT and FOXP1FL detected neither FOXP1-related fusions nor FOXP1 mutations. Further bioinformatic analysis of RNA-sequencing data retrieved a set of genes, which may comprise direct or non-direct targets of FOXP1NT, potentially implicated in disease progression. In summary, our findings point to a dual mechanism through which FOXP1 is implicated in B-cell lymphomagenesis. We hypothesize that the primary t(3; 14)(p13; q32)/IGH-FOXP1 activates expression of the FOXP1FL protein with potent oncogenic activity, whereas the secondary non-IG rearrangements of FOXP1 promote expression of the FOXP1NT proteins, likely driving progression of disease.
Background
Adrenocortical tumors comprise frequent adenomas (ACA) and rare carcinomas (ACC). Human cytochrome P450 2W1 (CYP2W1) is highly expressed in some cancers holding the potential to activate certain drugs into tumor cytotoxins.
Objective
To investigate the CYP2W1 expression in adrenal samples and its relationship with clinical outcome in ACC.
Material and Methods
CYP2W1 expression was investigated by qRT-PCR in 13 normal adrenal glands, 32 ACA, 25 ACC, and 9 different non-adrenal normal tissue samples and by immunohistochemistry in 352 specimens (23 normal adrenal glands, 33 ACA, 239 ACC, 67 non-adrenal normal or neoplastic samples).
Results
CYP2W1 mRNA expression was absent/low in normal non-adrenal tissues, but high in normal and neoplastic adrenal glands (all P<0.01 vs non-adrenal normal tissues). Accordingly, CYP2W1 immunoreactivity was absent/low (H-score 0–1) in 72% of non-adrenal normal tissues, but high (H-score 2–3) in 44% of non-adrenal cancers, in 65% of normal adrenal glands, in 62% of ACAs and in 50% of ACCs (all P<0.001 vs non-adrenal normal tissues), being significantly increased in steroid-secreting compared to non-secreting tumors. In ACC patients treated with mitotane only, high CYP2W1 immunoreactivity adjusted for ENSAT stage was associated with longer overall survival and time to progression (P<0.05 and P<0.01, respectively), and with a better response to therapy both as palliative (response/stable disease in 42% vs 6%, P<0.01) or adjuvant option (absence of disease recurrence in 69% vs 45%, P<0.01).
Conclusion
CYP2W1 is highly expressed in both normal and neoplastic adrenal glands making it a promising tool for targeted therapy in ACC. Furthermore, CYP2W1 may represent a new predictive marker for the response to mitotane treatment.
The genetic mechanisms underlying adrenocortical tumor development are still largely unknown. We used high-resolution single nucleotide polymorphism microarrays (Affymetrix SNP 6.0) to detect copy number alterations (CNAs) and copy-neutral losses of heterozygosity (cnLOH) in 15 cortisol-secreting adrenocortical adenomas with matched blood samples. We focused on microalterations aiming to discover new candidate genes involved in early tumorigenesis and/or autonomous cortisol secretion. We identified 962 CNAs with a median of 18 CNAs per sample. Half of them involved noncoding regions, 89% were less than 100 kb, and 28% were found in at least two samples. The most frequently gained regions were 5p15.33, 6q16.1, 7p22.3-22.2, 8q24.3, 9q34.2-34.3, 11p15.5, 11q11, 12q12, 16q24.3, 20p11.1-20q21.11, and Xq28 (>= 20% of cases), most of them being identified in the same three adenomas. These regions contained among others genes like NOTCH1, CYP11B2, HRAS, and IGF2. Recurrent losses were less common and smaller than gains, being mostly localized at 1p, 6q, and 11q. Pathway analysis revealed that Notch signaling was the most frequently altered. We identified 46 recurrent CNAs that each affected a single gene (31 gains and 15 losses), including genes involved in steroidogenesis (CYP11B1) or tumorigenesis (CTNNB1, EPHA7, SGK1, STIL, FHIT). Finally, 20 small cnLOH in four cases affecting 15 known genes were found. Our findings provide the first high-resolution genome-wide view of chromosomal changes in cortisol-secreting adenomas and identify novel candidate genes, such as HRAS, EPHA7, and SGK1. Furthermore, they implicate that the Notch1 signaling pathway might be involved in the molecular pathogenesis of adrenocortical tumors.
Deep phenotypical characterization of human CD3\(^{+}\)CD56\(^{+}\) T cells by mass cytometry

CD56\(^{+}\) T cells are a group of pro‐inflammatory CD3\(^{+}\) lymphocytes with characteristics of natural killer cells, being involved in antimicrobial immune defense. Here, we performed deep phenotypic profiling of CD3\(^{+}\)CD56\(^{+}\) cells in peripheral blood of normal human donors and individuals sensitized to birch‐pollen or/and house dust mite by high‐dimensional mass cytometry combined with manual and computational data analysis. A co‐regulation between major conventional T‐cell subsets and their respective CD3\(^{+}\)CD56\(^{+}\) cell counterparts appeared restricted to CD8\(^{+}\), MAIT, and TCRγδ\(^{+}\) T‐cell compartments. Interestingly, we find a co‐regulation of several CD3\(^{+}\)CD56\(^{+}\) cell subsets in allergic but not in healthy individuals. Moreover, using FlowSOM, we distinguished a variety of CD56\(^{+}\) T‐cell phenotypes demonstrating a hitherto underestimated heterogeneity among these cells. The novel CD3\(^{+}\)CD56\(^{+}\) subset description comprises phenotypes superimposed with naive, memory, type 1, 2, and 17 differentiation stages, in part represented by a phenotypical continuum. Frequencies of two out of 19 CD3\(^{+}\)CD56\(^{+}\) FlowSOM clusters were significantly diminished in allergic individuals, demonstrating less frequent presence of cells with cytolytic, presumably protective, capacity in these donors consistent with defective expansion or their recruitment to the affected tissue. Our results contribute to defining specific cell populations to be targeted during therapy for allergic conditions.
Objective Refractory coeliac disease (RCD) is a potentially hazardous complication of coeliac disease (CD). In contrast to RCD type I, RCD type II is a precursor entity of enteropathy-associated T-cell lymphoma (EATL), which is associated with clonally expanding T-cells that are also found in the sequentially developing EATL. Using high-throughput sequencing (HTS), we aimed to establish the small-intestinal T-cell repertoire (TCR) in CD and RCD to unravel the role of distinct T-cell clonotypes in RCD pathogenesis. Design DNA extracted from duodenal mucosa specimens of controls (n=9), active coeliacs (n=10), coeliacs on a gluten-free diet (n=9), RCD type I (n= 8), RCD type II (n= 8) and unclassified Marsh I cases (n= 3) collected from 2002 to 2013 was examined by TCR beta-complementarity- determining regions 3 (CDR3) multiplex PCR followed by HTS of the amplicons. Results On average, 106 sequence reads per sample were generated consisting of up to 900 individual TCR beta rearrangements. In RCD type II, the most frequent clonotypes (ie, sequence reads with identical CDR3) represent in average 42.6% of all TCR beta rearrangements, which was significantly higher than in controls (6.8%; p<0.01) or RCD type I (6.7%; p<0.01). Repeat endoscopies in individual patients revealed stability of clonotypes for up to several years without clinical symptoms of EATL. Dominant clonotypes identified in individual patients with RCD type II were unique and not related between patients. CD-associated, gliad-independent CDR3 motifs were only detectable at low frequencies. Conclusions TCR beta-HTS analysis unravels the TCR in CD and allows detailed analysis of individual TCR beta rearrangements. Dominant TCR beta sequences identified in patients with RCD type II are unique and not homologous to known gliadin-specific TCR sequences, supporting the assumption that these clonal T-cells expand independent of gluten stimulation.
Iron as the concert master in the pathogenic orchestra playing in sporadic Parkinson's disease

About 60 years ago, the discovery of a deficiency of dopamine in the nigro-striatal system led to a variety of symptomatic therapeutic strategies to supplement dopamine and to substantially improve the quality of life of patients with Parkinson's disease (PD). Since these seminal developments, neuropathological, neurochemical, molecular biological and genetic discoveries contributed to elucidate the pathology of PD. Oxidative stress, the consequences of reactive oxidative species, reduced antioxidative capacity including loss of glutathione, excitotoxicity, mitochondrial dysfunction, proteasomal dysfunction, apoptosis, lysosomal dysfunction, autophagy, suggested to be causal for ɑ-synuclein fibril formation and aggregation and contributing to neuroinflammation and neural cell death underlying this devastating disorder. However, there are no final conclusions about the triggered pathological mechanism(s) and the follow-up of pathological dysfunctions. Nevertheless, it is a fact, that iron, a major component of oxidative reactions, as well as neuromelanin, the major intraneuronal chelator of iron, undergo an age-dependent increase. And ageing is a major risk factor for PD. Iron is significantly increased in the substantia nigra pars compacta (SNpc) of PD. Reasons for this finding include disturbances in iron-related import and export mechanisms across the blood-brain barrier (BBB), localized opening of the BBB at the nigro-striatal tract including brain vessel pathology. Whether this pathology is of primary or secondary importance is not known. We assume that there is a better fit to the top-down hypotheses and pathogens entering the brain via the olfactory system, then to the bottom-up (gut-brain) hypothesis of PD pathology. Triggers for the bottom-up, the dual-hit and the top-down pathologies include chemicals, viruses and bacteria. If so, hepcidin, a regulator of iron absorption and its distribution into tissues, is suggested to play a major role in the pathogenesis of iron dyshomeostasis and risk for initiating and progressing ɑ-synuclein pathology. The role of glial components to the pathology of PD is still unknown. However, the dramatic loss of glutathione (GSH), which is mainly synthesized in glia, suggests dysfunction of this process, or GSH uptake into neurons. Loss of GSH and increase in SNpc iron concentration have been suggested to be early, may be even pre-symptomatic processes in the pathology of PD, despite the fact that they are progression factors. The role of glial ferritin isoforms has not been studied so far in detail in human post-mortem brain tissue and a close insight into their role in PD is called upon. In conclusion, "iron" is a major player in the pathology of PD. Selective chelation of excess iron at the site of the substantia nigra, where a dysfunction of the BBB is suggested, with peripherally acting iron chelators is suggested to contribute to the portfolio and therapeutic armamentarium of anti-Parkinson medications.
Vor fast 100 Jahren postulierte Paul Ehrlich ein körpereigenes System, das in der Lage ist, Tumorentstehung zu erkennen und zu eliminieren. Die Weiterentwicklung dieser Hypothese führte in den Folgejahren zum Modell der „immunosurveillance“, einer angeborenen Wachfunktion des Immunsystems, welche im Regelfall die Entstehung von manifesten Tumoren verhindern kann. Neben zellulären Bestandteilen wie Makrophagen und natürlichen Killerzellen kommt dabei eine entscheidende Bedeutung den B-Lymphozyten mit ihrer Fähigkeit zur Produktion eines variablen Antikörperrepertoires zu. Diese angeborene, IgM-vermittelte, humorale Abwehrfunktion richtet sich insbesondere auch gegen glykopeptidische Oberflächenantigene maligner körpereigener Zellen. Allerdings scheint sich in seltenen Einzelfällen ein Tumorprozess durch Fluchtmechanismen dem Zugriff der Immunabwehr entziehen zu können. Die intensiven Wechselwirkungen von Immunsystem und Tumorwachstum sind Ziel weitreichender Forschung geworden, angetrieben von der Hoffnung, in naher Zukunft neue immunologische Therapieverfahren gegen maligne Tumoren aufbieten zu können. Bisher wenig untersucht ist allerdings die lokale Situation der humoralen Immunität innerhalb von Tumorgewebe. Vor diesem Hintergrund wurde in der vorliegenden Arbeit eine Sequenzanalyse des exprimierten Immunglobulin-Repertoires in situ, also aus gewebeständigen tumorinfiltrierenden B-Lymphozyten und solchen aus tumorfreiem Gewebe vorgenommen. Als Ausgangsmaterial diente Gewebe aus Magenkarzinomen und tumorfreier Magenschleimhaut des Magenantrums von acht Patienten mit fortgeschrittener Tumorerkrankung. Adenokarzinome des Magens stellen eine multifaktoriell bedingte, weltweit häufig auftretende Tumorentität mit besonders schlechter Prognose dar. In der Tat zeigen sich in den vorliegenden Ergebnissen signifikante Unterschiede zwischen Tumor und Antrum in den variablen Schwerkettengenen der gewebeständigen B-Lymphozyten. Diese betreffen sowohl die IgVH- Familienzugehörigkeit als auch die Mutationsraten und Mutationsmuster der einzelnen Sequenzen. Die entscheidenden Beobachtungen im Rahmen dieser Arbeit sind der in situ- Nachweis von naiven, nicht immunitätsgereiften Immunglobulinen der primären Immunantwort, insbesondere der mit Antitumoraktivität in Verbindung gebrachten IgVH3-Familie innerhalb des untersuchten Antrumgewebes sowie deren Fehlen innerhalb des Tumorprozesses. Der Nachweis affinitätsgereifter Immunglobuline innerhalb des Tumorprozesses legt eine zwar intensive, aber letztendlich ineffektive Auseinandersetzung des Immunsystems mit dem sich offenbar in diesem Stadium nicht mehr als ausreichend immunogenen erweisenden Tumorprozess nahe. Diese Ergebnisse heben die entscheidende Bedeutung der naiven Immunität für die Verhinderung von Tumoren (Immunüberwachung) hervor. Unklar sind allerdings nach wie vor die Escape-Mechanismen, die es einem Tumor erlauben, sich dieser primären Immunantwort zu entziehen. Weitere Forschungsarbeit im Bereich naiver Immunglobuline mit Antitumoraktivität, wie beispielsweise dem selektiv an Magenkarzinomzellen bindenden und Apoptose auslösenden Antikörper SC-1, könnte dazu beitragen, zukünftig völlig neue adjuvante immunologische Therapieverfahren in der Tumortherapie zu etablieren.
Hibernoma is a rare benign lipomatous tumor showing differentiation of brown fatty tissue. To the author’s best knowledge, there is no known case of malignant transformation or metastasis. Due to their slow, noninfiltrating growth hibernomas are often an incidental finding in the third or fourth decade of life. The vast majority are located in the thigh, neck, and periscapular region. A diagnostic workup includes ultrasound and contrast-enhanced MRI. Differential diagnosis is benign lipoma, well-differentiated liposarcoma, and rhabdomyoma. An incisional biopsy followed by marginal resection of the tumor is the standard of care, and recurrence after complete resection is not reported. The current paper presents diagnostic and intraoperative findings of a hibernoma of the upper arm and reviews similar reports in the current literature.
Erste tumorassoziierte Veränderungen finden im Glykosilierungsmuster von Glykoproteinen und Glykolipiden statt. Die dabei entstehenden tumorassoziierten Carbohydrat-Antigene sind prominente Zielstrukturen der natürlichen Tumorimmunität (Immune Surveillance) und gewinnen in der Onkologie als immunogene Epitope zunehmend an Bedeutung. Der humane monoklonale IgM-Antikörper SAM-6 ist Teil der tumorspezifischen Immunität. Er wurde mit Hilfe der konventionellen Hybridomatechnologie direkt aus einem an Magenkarzinom erkrankten Menschen isoliert. Neben der Erforschung seines außergewöhnlichen Apoptose-mechanismus konnte innerhalb dieser Arbeit eine Zielstruktur des Antikörpers identifiziert und charakterisiert werden. Der humane monoklonale IgM-Antikörper SAM-6 bindet an eine neue Isoform des Hitzeschockproteins GRP78 (GRP78SAM-6). Das Antigen wurde über mehrstufige chromatographische Verfahren aus Tumorzellmembranextrakten aufgereinigt und nach tryptischen Verdau über die Methode des Peptidmassen-Fingerprinting eindeutig als humanes GRP78 identifiziert. Die auf der Zellmembran lokalisierte Variante des GRP78 besitzt ein Molekulargewicht von 82 kD und wird auf vielfältigen Tumorgeweben stabil exprimiert. GRP78SAM-6 liegt parallel zur 78 kD-Wildtyp-Variante co-exprimiert vor und konnte im Gegensatz zur Wildtyp-Variante nicht auf gesundem Gewebe nachgewiesen werden. Bei der SAM-6-spezifischen Variante des GRP78 handelt es sich um eine posttranslational modifizierte Form des GRP78, die spezifisch auf der Zellmembran lokalisiert ist, nicht jedoch intrazellulär zu finden ist. Sie unterscheidet sich durch zusätzliche Glykosilierungen vom GRP78-Wildtypen, wobei O-glykosidisch verknüpfte Glykane für die Bindung und die Reaktion mit dem SAM-6 Antikörper essentiell sind. Der SAM-6-Rezeptor stellt eine tumorspezifische Isoform des Hitzeschockproteins GRP78 dar, deren O-glykosilierte Carbohydrat-Regionen als Epitop fungieren. Durch die Bindung an GRP78SAM-6 hemmt der Antikörper SAM-6 in vitro als auch in vivo konzentrationsabhängig das Wachstum von Magen- und Pankreaskarzinomzellen und induziert eine neue Art des apoptotischen Zelltodes, die sog. Lipoptose. Es handelt sich um einen durch den Antikörper vermittelten direkten Effekt, der sich ausschließlich auf malignes Gewebe beschränkt. Schlüsselpunkt der apoptotischen Wirkung ist die Akkumulation zytotoxischer Mengen an Cholesterol und Triglyceridestern, die nach Bindung an den Antikörper in Form von Lipoprotein-Partikeln in die Tumorzelle gelangen. Der pentamere IgM-Antikörper bindet neben membranständigem GRP78SAM-6 der Tumorzelle, die ApoB 100-haltigen Lipoproteine VLDL und LDL. Insbesondere oxidativ modifizierte Formen des LDL (oxLDL) zeigten dabei die höchste Bindungsaffinität zum SAM-6 Antikörper. In deren Anwesenheit war ein maximaler lipotoxischer Effekt zu beobachten. Im Rahmen dieser Arbeit ist es gelungen, weite Teile des Lipoptose-Mechanismus aufzuklären. Eigenen Immunfluoreszenzstudien zufolge wird der SAM-6 Antikörper über rezeptorvermittelte Endozytose internalisiert. Die GRP78-vermittelte Internalisierung von oxLDL-beladenem Antikörper scheint daher plausibel und für die tödliche Anhäufung der Lipide verantwortlich zu sein. Die SAM-6-induzierte Apoptose verläuft anschließend über einen spezifischen Signalweg, der Gemeinsamkeiten mit dem intrinsischen Signalweg aufweist, jedoch wie beim extrinsischen Signalweg über externe pro-apoptotische Liganden angeregt wird. Infolge der unphysiologisch hohen intrazellulären Konzentration an oxLDL kommt es zur Induktion einer Caspasenkaskade, die nach der initialen Freisetzung von Cytochrom C aus den Mitochondrien über die Initiatorcaspasen 8 und 9 verläuft und letztendlich durch die Aktivierung der terminalen Caspasen 3 und 6 den apoptotischen Zelltod einleitet. Die Entdeckung von extrazellulär exprimiertem GRP78 auf Tumorzellen bietet die Möglichkeit neuer Therapieansätze in der Onkologie. Die SAM-6-spezifische Variante des GRP78 bietet insbesondere die Möglichkeit eines gezielten Angriffs auf die Tumorzelle, ohne gesunde Zellen zu tangieren. Sie wird auf Tumorgeweben verschiedenster Ätiologie stabil exprimiert und infolge ihres tumorspezifischen Auftretens zur optimalen Zielstruktur der natürlichen körpereigenen Immunantwort gegen Tumore. Der natürliche IgM-Antikörper ist Teil der natürlichen Immunität. Diese verfügt über ein breites Repertoire an Rezeptoren und garantiert die permanente Überwachung und Reaktion gegen modifizierte körpereigene Zellen. Sie ist dafür verantwortlich, dass Tumore sich nur in Ausnahmefällen manifestieren.
The massive infiltration of lymphocytes into the skin is a hallmark of numerous human skin disorders. By co-culturing murine keratinocytes with splenic T cells we demonstrate here that T cells affect and control the synthesis and secretion of chemokines by keratinocytes. While pre-activated CD8\(^+\)T cells induce the synthesis of CXCL9 and CXCL10 in keratinocytes and keep in check the synthesis of CXCL1, CXCL5, and CCL20, keratinocytes dampen the synthesis of CCL3 and CCL4 in pre-activated CD8\(^+\)T cells. One key molecule is IFN-γ that is synthesized by CD8\(^+\)T cells under the control of NFATc1 and NFATc2. CD8\(^+\)T cells deficient for both NFAT factors are unable to induce CXCL9 and CXCL10 expression. In addition, CD8\(^+\)T cells induced numerous type I IFN-inducible “defense genes” in keratinocytes encoding the PD1 and CD40 ligands, TNF-α and caspase-1. The enhanced expression of type I IFN-inducible genes resembles the gene expression pattern at the dermal/epidermal interface in lichen planus, an inflammatory T lymphocyte-driven skin disease, in which we detected the expression of CXCL10 in keratinocytes in close vicinity to the infiltration front of T cells. These data reflect the multifaceted interplay of lymphocytes with keratinocytes at the molecular level.
Drug resistance is a significant obstacle in cancer treatment and therefore a frequent subject of research. Developed or primary resistance limits the treatment success of inhibitors of the B cell receptor (BCR) pathway in mantle cell lymphoma (MCL) patients. Recent research has highlighted the role of the nuclear factor-kappa B (NF kappa B) pathway in the context of resistance to BCR inhibitors in MCL. In this study, we analyzed the dependency of MCL cell lines on NF kappa B signaling and illustrated the ability of CD40L to activate the alternative NF kappa B pathway in MCL. This activation leads to independency of classical NF kappa B signaling and results in resistance to BCR inhibitors. Therefore, ligands (such as CD40L) and their activation of the alternative NF kappa B pathway have a major impact on the drug response in MCL. Furthermore, this study indicates a protective role for cells expressing specific ligands as microenvironmental niches for MCL cells and underlines the significance of therapeutically targeting alternative NF kappa B signaling in MCL.
In contrast to other haematological malignancies, targeted immunotherapy has not entered standard treatment regimens for de novo or relapsed multiple myeloma (MM) yet. While a number of IgG-formatted monoclonal antibodies are currently being evaluated in clinical trials in MM, our study aimed to investigate whether the fully human IgM monoclonal antibody PAT-SM6 that targets a tumour-specific variant of the heat shock protein GRP78 might be an attractive candidate for future immunotherapeutic approaches. We here show that GRP78 is stably and consistently expressed on the surface on tumour cells from patients with de novo, but also relapsed MM and that binding of PAT-SM6 to MM cells can specifically exert cytotoxic effects on malignant plasma cells, whereas non-malignant cells are not targeted. We demonstrate that the induction of apoptosis and, to a lesser extent, complement dependent cytotoxicity is the main mode of action of PAT-SM6, whereas antibody dependent cellular cytotoxicity does not appear to contribute to the cytotoxic properties of this antibody. Given the favourable safety profile of PAT-SM6 in monkeys, but also in a recent phase I trial in patients with malignant melanoma, our results form the basis for a planned phase I study in patients with relapsed MM.
Humane oder humanisierte monoklonale Antikörper haben sich in den letzten zehn Jahren als Arzneimittel etabliert. Sie sind hochspezifisch und zeigen in ihrer Anwendung im Vergleich zu konventionellen Therapeutika viel weniger Nebenwirkungen. In den 80er Jahren gelang es am Pathologischen Institut der Universität Würzburg eine Reihe von humanen Antikörpern aus Patienten zu isolieren, die hochspezifisch mit malignen Zellen reagieren und diese sowohl in vitro als auch im experimentellen Tiermodel selektiv durch Induktion von Apoptose töten. Um die Wirkungsweise von monoklonalen Antikörpern in der Krebstherapie zu erhöhen, werden die meisten in Kombination mit herkömmlichen Methoden, wie Chemotherapie, eingesetzt. Die ideale Therapieform sind hinsichtlich der Nebenwirkungen sog. Cocktails aus verschiedenen monoklonalen Antikörpern. Allerdings sind die Studien hierzu noch wenig fortgeschritten. Das Ziel dieser Arbeit war es, in präklinischen Versuchsreihen den Einsatz verschiedener tumorspezifischer humaner monoklonaler Antikörper als Cocktail und in Kombination mit Chemotherapie zu evaluieren. Hierzu wurden neun Antikörper in 32 verschiedenen Antikörperkombinationen hinsichtlich ihrer Auswirkungen auf die in vitro Proliferation einer Pankreaskarzinom-Zellinie untersucht. In Immunfluoreszenz-Aufnahmen ließ sich zeigen, dass kombinierte Antikörper an unterschiedlichen Stellen an der Zelle binden, was eindeutig auf verschiedene Zielstrukturen hinweist. Einige werden dabei endozytiert, während andere auf der Zellmembran bleiben. Interessanterweise ließen sich Kombinationen identifizieren, deren antiproliferative Wirkung sowohl additiv als auch synergistisch ist, das heißt größer als die Summe ihrer Einzelaktivitäten. Wurden Antikörper mit Zytostatika (5-Flurouracil) kombiniert, so ließen sich ebenfalls synergistische Effekte beobachten. In FACS-Analysen zeigt sich ein gesteigertes Bindungsverhalten der Antikörper, wenn die Zellen mit 5-FU vorinkubiert wurden. Zusammenfassend bestätigen die Ergebnisse dieser Arbeit die Beobachtung, dass die Wirkung humaner monoklonaler Antikörper in Kombination mit Chemotherapie erhöht werden kann. Für die Zukunft humaner Antikörper als Therapiemittel gegen maligne Erkrankungen mag allerdings noch wichtiger sein, dass Antiköper in Cocktails tatsächlich synergistische Wirkung zeigen können.
Fibrosis, neurodegeneration, and cerebral angiomatosis (FINCA, MIM#618278) is a rare clinical condition caused by bi‐allelic variants in NHL repeat containing protein 2 (NHLRC2, MIM*618277). Pulmonary disease may be the presenting sign and the few patients reported so far, all deceased in early infancy. Exome sequencing was performed on patients with childhood interstitial lung disease (chILD) and additional neurological features. The chILD‐EU register database and an in‐house database were searched for patients with NHLRC2 variants and clinical features overlapping FINCA syndrome. Six patients from three families were identified with bi‐allelic variants in NHLRC2. Two of these children died before the age of two while four others survived until childhood. Interstitial lung disease was pronounced in almost all patients during infancy and stabilized over the course of the disease with neurodevelopmental delay (NDD) evolving as the key clinical finding. We expand the phenotype of FINCA syndrome to a multisystem disorder with variable severity. FINCA syndrome should also be considered in patients beyond infancy with NDD and a history of distinct interstitial lung disease. Managing patients in registers for rare diseases helps identifying new diagnostic entities and advancing care for these patients.
While numerous experiments on NFAT were already performed with CD4+ T cells showing defective cytokine release and a reduced T helper cell development, no detailed studies existed for CD8+ T cells. From this point, we wanted to examine the impact of NFATc1 and c2 on the physiological functions of CD8+ T cells in vitro and in vivo. Therefore, we used a murine infection model with the bacteria Listeria monocytogenes and mice in which NFATc1 was specifically depleted in the T cell compartment.
Our first in vitro studies showed a typical NFATc1 and c2 nuclear translocation and changes on mRNA levels upon T cell activation similarly in CD4+ as well as in CD8+ T cells extracted from wild type mice. NFAT nuclear translocation is important for target gene activation and generation of effector functions. Stimulated T cell populations lacking NFATc1 and/or NFATc2 showed a markedly decreased expression of Th1/Tc1 cytokines, as e.g. IL 2 and IFNγ being important for the clearance of intracellular pathogens. From our in vitro model for the generation of allogenically reactive cytotoxic CD8+ T cells, we revealed a decreased killing and lytic granule-release capacity in Nfatc1 inactivated CD8+ T cells whereas NFATc2-/- cytotoxic T cells did not show an altered cytotoxic response compared to wild type cells.
Interestingly, we found lytic granules accumulated and mitochondria not getting translocated to the immunological synapse upon re-stimulation in NFATc1-deficient CD8+ T cells. Together with results showing the CsA insensitivity of the CTL killing/degranulation capacities, we assume that some major cellular processes are affected by NFATc1 which are not directly linked to the TCR-induced signal transduction cascade.
We also showed the importance of NFATc1 in T cells during intracellular infections with the bacteria Listeria monocytogenes in an in vivo mouse model. After five days, only few bacteria were detected in wt mice whereas high amounts of Listeria particles were extracted from livers of Nfatc1fl/fl x Cd4 cre mice. Although the reactivity towards the pathogen was similar in both groups, a decreased cytokine expression in NFATc1-/- CD8+ T cells was observed together with an altered memory cell generation.
Our results show the importance of NFATc1 in CD8+ T cells and give some clue for a possible connection to other basal cellular functions, as e.g. the formation of an immunological synapse.
