Bei Morbus Fabry handelt es sich um eine X-chromosomal rezessiv vererbbare lysosomale Speichererkrankung. Im Vordergrund der kardialen Beteiligung stehen eine progrediente Herzinsuffizienz, bedingt durch eine linksventrikuläre Hypertrophie mit kardialer Fibrosierung, sowie eine Mitbeteiligung des Reizleitungssystems.
Bei 150 Patienten wurden im Zeitraum von 2001-2009 neben einer klinischen Untersuchung ein EKG, eine Echokardiographie, ein Belastungs-EKG und teilweise auch eine Magnetresonanztomographie durchgeführt. Zum Vergleich der Patientenentwicklung wurde jeweils das jüngste Follow-up Ergebnis mit den Baseline-Daten verglichen.
Es konnte eine signifikante Korrelation zwischen der QRS-Dauer und der Wandstärke in der Echokardiographie und der Magnetresonanztomographie eindeutig nachgewiesen werden. Eine myokardiale Fibrose ist bei normalen Ruhe-EKG-Parametern nahezu auszuschließen. In der Untersuchung des Langzeit-EKGs fanden sich bei einigen Patienten höhergradige ventrikuläre Rhythmusstörungen, welche als erhöhtes individuelles Risiko und als bedeutender Faktor der Sterblichkeit bei Morbus Fabry zu werten sind.
Die biochemische Diagnostik von Phäochromozytomen und Paragangliomen (PPGL) basiert auf einem gesteigerten Katecholaminmetabolismus in dessen Folge es zu erhöhten Plasmametanephrinkonzentrationen (Normetanephrin NMN und/oder Metanephrin MN) kommt. Die variable endokrine Aktivität der Tumoren sowie Hormonzentrationen im Nanomol-pro-Liter-Bereich stellen hohe Anforderungen an analytische Messmethoden. Nicht oder spät diagnostizierte PPGL können lebensbedrohliche katecholaminerge Krisen verursachen. Demzufolge sind präzise Nachweisverfahren und validierte Messmethoden für die biochemische Diagnostik unerlässlich. Inwiefern die Messverfahren enzymgekoppelter Immunoassay (EIA) und Flüssigkeitschromatographie mit Tandem-Massenspektrometrie (LC-MS/MS) zur Analyse von Plasmametanephrinen in gleichem Maße geeignet sind, ist aufgrund der kontroversen Studienlage unklar.
Hierfür wurde eine Substudie im Rahmen der prospektiven monoaminproduzierenden Tumor (PMT)-Studie an fünf europäischen Zentren durchgeführt. Es wurden 341 Patienten (174 Männer, 167 Frauen), darunter 54 Patienten mit PPGL eingeschlossen. Die Blutproben wurden unter Standardbedingungen entnommen und mit beiden Messmethoden in spezialisierten Zentren analysiert. Für den EIA zeigte sich ein negativer Bias von
60% für NMN und 39% für MN. Dieser führte unter Verwendung oberer Grenzwerte nach Herstellerangaben zu einer Sensitivität von 74,1% und einer Spezifität von 98,9%. Für LC-MS/MS wurde eine signifikant höhere Sensitivität von 98,1% und eine Spezifität von 99,7% gezeigt. Die Berechnung der area under the curve (AUC) im Zuge der receiver-operating-characteristic (ROC)-Kurven-Analyse indizierte jedoch eine vergleichbar hohe diagnostische Testleistung für EIA (0,993) und LC-MS/MS (0,985). Durch eine Bias-korrigierte Grenzwertoptimierung stieg die Sensitivität des EIAs auf 96,2% bei einer Spezifität von 95,1% und führte dadurch zu einer vergleichbaren diagnostischen Testqualität wie
LC-MS/MS.
Fazit: Bei diesem kommerziell erhältlichen EIA resultiert die Verwendung oberer Grenzwerte nach Herstellerangaben in einer insuffizienten Sensitivität. Dies birgt das Risiko, Patienten mit PPGL nicht zu diagnostizieren. Eine Rekalibrierung des EIAs sowie eine Validierung der Referenzintervalle sind erforderlich.
Die Herzinsuffizienz gehört, trotz verbesserter Diagnostik und Therapie, zu den häufigsten Todesursachen in Deutschland und ist nach wie vor eine progrediente Erkrankung mit hoher Morbidität.
Kompensationsmechanismen des Herzens dienen zunächst der Aufrechterhaltung einer ausreichenden Herzleistung, haben jedoch im fortgeschrittenen Stadium der Erkrankung sogar ungünstige Effekte. Die Therapie umfasst nicht-medikamentöse und medikamentöse Ansätze, die in der Regel kombiniert zum Einsatz kommen – angepasst an Schweregrad und Akuität der Erkrankung. Die Pharmakotherapie bildet in der Regel die Basis der Herzinsuffizienztherapie. Trotz eindrucksvoller Erfolge der medikamentösen Therapiestrategien im Hinblick auf Symptomverbesserung und Prognose ist in vielen Fällen der Progress der Erkrankung dadurch nicht aufzuhalten. Die nicht-medikamentösen Therapieformen einer Herzinsuffizienz sollen daher immer flankierend zum Einsatz kommen und reichen von körperlicher Bewegung, Risikofaktorenmanagement, multidisziplinärer Betreuung über die Implantation von kardialen Resynchronisierungssystemen oder komplexen herzchirurgischen Maßnahmen bis hin zur Herztransplantation1.
Zur Diagnostik einer Herzinsuffizienz finden sowohl apparative als auch laborchemische Methoden ihre Anwendung. Sogenannte Biomarker, d.h. in der Regel im Blut nachweisbare Faktoren helfen, eine Aussage über die Schwere der Herzinsuffizienz und die Prognose zu treffen.
Beim „Syndrom Herzinsuffizienz“ handelt es sich um eine Systemerkrankung. Das Risiko einer (Re-)Hospitalisierung und die Mortalität aufgrund einer Herzinsuffizienz sind deutlich erhöht. In der Pathogenese und Progression der Herzinsuffizienz spielt die Inflammation eine zentrale Rolle. Diverse Möglichkeiten der Detektion einer Inflammation stehen dem Mangel des therapeutischen Eingreifens gegenüber. Aktuelle Studien zur anti-inflammatorischen Therapie konnten bisher keine Verringerung der Hospitalisierungsrate oder Mortalitätsreduktion zeigen.
In der vorliegenden Arbeit soll untersucht werden, ob eine Kombination aus Markern der Herzinsuffizienz (NT-proBNP) mit Inflammationsmarkern eine bessere prognostische Abschätzung erlaubt und welche Biomarker-Kombination sinnvoll ist, um die Patienten mit einem erhöhten Risiko zu charakterisieren, um hier eine engmaschigere Betreuung zu initiieren.
Die Arbeitshypothese lautet daher, dass die Marker Prädiktoren für Tod und Rehospitalisierung bei Herzinsuffizienzpatienten sind und in Kombination die prognostische Aussagekraft verbessern. Außerdem wird angenommen, dass die Marker mit wichtigen Begleiterkrankungen des Herzinsuffizienzsyndroms assoziiert sind.
Die idiopathische dilatative Kardiomyopathie ist eine eher seltene Herzerkrankung (Inzidenz 8/100.000 Einwohner pro Jahr), jedoch eine der häufigsten Ursache für die Entstehung einer Herzinsuffizienz bei jüngeren Patienten und geht immer mit Veränderungen sowohl des humoralen als auch des zellulären Immunsystems einher.
Die der vorliegenden Arbeit zugrunde liegende ETiCS-Studie untersucht besonders die Rolle von 1-adrenergen Autoantikörpern bei der Ausbildung und Entwicklung einer Herzinsuffizienz sowie deren Einfluss auf den Verlauf der kardialen Pumpfunktion nach einem ersten akuten Myokardinfarkt (FAMI) oder einer ersten Episode einer akuten Myokarditis (AMitis).
Nach Studieneinschluss wurden diese beiden klar definierten Patientenkollektive über einen Zeitraum von 12 Monaten beobachtet. Im Fokus der vorliegenden Arbeit stand der Verlauf der mittels MRT erhobenen kardialen Funktionsdaten (LVEF, EDV, ESV, SV, HI und LV-Masse, Baseline und 12 Monats-Follow Up) und ihr Zusammenhang mit Biomarkern der Herzschädigung (CK/CK-MB) sowie der Ausbildung/dem Verlauf von 1-Autoantikörpern.
FAMI-Patienten wiesen innerhalb eines Jahres grundsätzlich eine Verbesserung der kardialen Pumpfunktion auf; Patienten mit erstem Hinterwandinfarkt zeigten im Vergleich zu Vorderwandinfarkt-Patienten bei generell besseren Ausgangswerten auch im Verlauf deutlich geringere Funktions- und Volumenänderungen. Patienten mit kleineren Myokardinfarkten (CK Werte <1000 U/l) zeigten, unabhängig von der Lokalisation, eine bessere Erholung der LV-Funktion, als Patienten mit größeren Herzinfarkten (CK Werte >1000 U/l). Bei den im Rahmen dieser Arbeit analysierten Infarktpatienten konnte allerdings kein wesentlicher Einfluss einer Ausbildung oder Gegenwart von herzspezifischen Autoantikörpern auf die Entwicklung der kardialen Pumpfunktion nachgewiesen werden.
AMitis-Patienten wiesen innerhalb eines Jahres ebenso grundsätzlich eine Verbesserung der kardialen Pumpfunktion auf; In dieser Patienten-Kohorte scheint die mittel- und langfristige Entwicklung der kardialen Funktionsparameter jedoch stark von der Ausbildung 1-adrenerger Autoantikörper beeinflusst zu sein: Antikörper-negative AMitis-Patienten zeigten zu jedem Beobachtungszeitpunkt deutlich bessere kardiale Funktionsparameter als Antikörper-positiv getestete AMitis-Patienten.
Dies bestätigt die initiale Hypothese der ETiCS-Studie und muss durch die abschließende Analyse aller ETiCS-Studienpatienten (erwartet 2019) noch bestätigt werden. Den Nachweis von 1-adrenergen Autoantikörpern in die Routine-Labordiagnostik einzuführen, erscheint aufgrund der Ergebnisse der vorliegenden Arbeit zumindest bei Myokarditispatienten sinnvoll. Bei einem Herzinfarkt sprechen die Ergebnisse der vorliegenden Arbeit jedoch nicht für einen Mehrwert eines routinemäßigen 1-Autoantikörper-Screenings.
Kardiovaskuläre Erkrankungen führen in Deutschland die Todesursachenstatistik an. Es gibt anerkannte Risikofaktoren, die veränderlich (Bluthochdruck, Diabetes, Übergewicht, Hyperlipidämie etc.) oder nicht veränderlich (Geschlecht, Alter, genetische Disposition) sein können. Bei langfristig erhöhten Risikofaktoren lässt sich eine Schädigung des Gefäßsystems feststellen, die sich u.a. durch eine zunehmende Versteifung der elastischen Gefäße ausdrückt.
Der Begriff „Arterielle Gefäßsteifigkeit“ umschreibt strukturelle und funktionelle Eigenschaften des arteriellen Gefäßsystems, die u.a. den pulsatilen aortalen Blutdruckes dämpfen und einen gleichmäßigeren peripheren Blutfluss ermöglichen („Windkesselfunktion“). Neben der physiologischen Alterung gibt es eine Reihe pathologischer Prozesse, die zu einer vorschnellen arteriellen Gefäßsteifigkeit führen: Hypertonus, Diabetes mellitus, Hyperlipidämie, Rauchen, verschiedenste Genpolymorphismen etc. Dort zeigen sich auch die Parameter der Gefäßsteifigkeit pathologisch verändert und ermöglichen auch prognostische Aussagen für kardiovaskuläre Erkrankungen bis hin zu allgemeinen Mortalität in verschiedensten Kollektiven.
Bedeutsamer Parameter der Arteriellen Gefäßsteifigkeit ist die Pulswellen-geschwindigkeit (PWV; in m/s). Diese beschreibt die Geschwindigkeit, mit der sich die Pulswelle entlang des Gefäßsystems ausbreitet. Diese Druckwelle besteht aus der initialen Welle (P1), die an Stellen mit Impedanzänderung (v.a. Übergang von elastischen Arterien zu denen vom muskulären Typ) reflektiert wird. Somit entsteht in der Druckkurve ein späterer zweiter Gipfel (P2) durch die sich überlagernde reflektierte Pulswelle. Es lassen sich in der Druckkurve folgende Augmentationsparameter bestimmen: Die Druckdifferenz zwischen P1 und P2 wird Augmentationsdruck (AP; in mmHg) genannt. Den Anteil von AP an der Blutdruckamplitude bezeichnet man als Augmentationsindex (AIx; in %).
Bekannt ist, dass sich die zentral-aortalen Blutdrücke (zentral systolischer Blutdruck CSBP; in mmHg), z.T. erheblich von den brachial gemessenen Blutdrücken (u.a. peripher systolischer Blutdruck PSBP; in mmHg) unterscheiden. Es ist jedoch der zentral-aortale Blutdruck, der z.B. die Nachlast des Herzen bestimmt. Der Begriff Pulsdruckamplifikation (PP Amp; in %) umschreibt das Verhältnis von peripheren Pulsdruck zu zentralem Pulsdruck (CPP; in mmHg).
Wir verwendeten zur nicht-invasiven Messung der Parameter der Arteriellen Gefäßsteifigkeit und zentralen Blutdrücke den Arteriograph (Tensiomed, Budapest, Ungarn). Die Messungen erfolgten oszillometrisch mittels einer Oberarm-Manschette. Aus der graphischen Auswertung der Druckkurve wurden die Parameter der arteriellen Gefäßsteifigkeit ermittelt. So wurde die PWV aus der Laufzeit der reflektierten Welle berechnet. Auch die übrigen Parameter der Augmentation (AIx, AP) und die zentral-aortalen Blutdrücke (CSBP, CPP, PP Amp) wurden graphisch bestimmt.
Die Arbeit umfasst Daten aus drei Kollektiven mit insgesamt 2.082 Probanden (68% Männer, mittleres Alter 54 Jahre). Zusätzlich wurden Daten zur kardiovaskulären Anamnese, Medikation und Tabakkonsum erhoben. Nach Ausschluss von Patienten mit bestehender kardiovaskulärer Erkrankung oder Risikofaktoren (KHK, Arterielle Hypertonie, Diabetes mellitus, Raucher, Hypercholesterinämie, bestehende kardiovaskuläre Medikation) gingen die Daten von 1.208 Probanden in die Ermittlung des alters- und geschlechtsspezifischen Verteilungsmusters der Gefäßsteifigkeitsparameter ein.
Zentrale Blutdrücke
Der zentrale systolische Blutdruck (CSBP) zeigte eine kontinuierliche altersabhängige Steigung, deutlicher ausgeprägt bei Frauen. Der CSBP lag in allen Altersklassen unter den jeweiligen peripheren systolischen Werten, es kam im Alter jedoch zu einer Angleichung. Ein signifikanter Geschlechtsunterschied fand sich nur in jüngeren Altersgruppen. Die Pulsdruckamplifikation (PP Amp) zeigte ebenso eine signifikante Altersabhängigkeit sowie Geschlechtsunterschiede in allen Altersgruppen.
Pulswellengeschwindigkeit
Nicht alle Ergebnisse unserer PWV-Messungen zeigten sich normalverteilt. Dies betraf v.a. Frauen ab dem 30. Lebensjahr, Männer ab dem 60. Lebensjahr sowie hohe PWV-Werte von 13-14 m/s. Zum Ausschluss dieser PWV-Werte außerhalb der Normalverteilung prüften wir verschiedene Qualitätskriterien. Wir entschieden uns für Anwendung eines Quotienten aus PWV-SD und PWV ≤ 10%. Somit verblieben für die alters- und geschlechtsspezifische Verteilung der PWV die Messwerte von 649 Personen (77% Männer). Es zeigte sich eine Assoziation mit dem Alter, bei Männer stärker (r²=0,198) als bei Frauen (r²= 0,178). Frauen wiesen in fast allen Altersgruppen höhere Messwerte auf als Männer. Vorher durch die Kriterien ausgeschlossene Probanden zeigten signifikant höhere PWV-Werte als kardiovaskulär Gesunde. In einer schrittweisen Regressionsanalyse blieben Alter, arterieller Mitteldruck, Geschlecht, Jugulum-Symphysen-Abstand sowie Gewicht als unabhängige Determinanten der PWV erhalten. Der oben erwähnte Geschlechtsunterschied stand im Kontrast zu bisherigen Veröffentlichungen, in denen aber andere Messmethoden verwendet wurden. Insgesamt war bei uns auch die Altersabhängigkeit geringer ausgeprägt als in vorherigen Publikationen. Da es bisher keine Veröffentlichung von oszillometrisch gemessenen Referenzwerten bei kardiovaskulär gesunden Erwachsenen gibt, war ein direkter Vergleich nicht möglich. Vergleiche mit anderen nicht-invasiven Messmethoden offenbarten jedoch Abweichungen gerade im höheren Alter sowie bei Frauen. Eine Erklärung ist die Schwierigkeit der graphischen Analyse, gerade dann, wenn die reflektierte Welle frühzeitig am Messort eintrifft. Mehrere Autoren äußerten zudem Bedenken, dass der Reflexionsort altersabhängig variiere. Somit sei eine Messung an nur einem Standort nicht für eine zuverlässige Messung geeignet.
Augmentationsparameter
Bei AP und aortalem AIx zeigte sich bei uns ein altersabhängiger Anstieg: Bei beiden Werten war der Verlauf bei Frauen im jüngeren Alter steiler, ab dem 50. Lebensjahr fanden sich nur noch geringfügig steigende Werte. Bei den Männern hingegen gab es eine durchgängige Steigung bis in hohe Alter. Frauen zeigten in jedem Alter bei beiden Parametern durchgehend höhere Werte. Signifikant beeinflussend waren Alter, Körpergröße, Herzfrequenz, arterieller Mitteldruck sowie das Geschlecht. Dies ist konstant zu vorigen Publikationen, lediglich der konstante Anstieg bei Männern bei uns steht im Gegensatz zu vorbeschriebenen logarithmischen Verläufen.
Risikoscores
Zur individuellen kardiovaskulären Risikoabschätzung wurden verschiedene Score-Systeme entwickelt. Durch nicht-pharmakologische und pharmakologische Maßnahmen wird insbesondere bei Patienten mit hohem Risiko versucht, die Manifestation kardiovaskulärer Erkrankungen zu verhindern.
Risikoscores (Framingham Risk Score, ESC Score, Reynolds Risk Score, Procam Risikoscore) wurden bei insgesamt 1434 Probanden (78% Männer, mittleres Alter 51 Jahre) ermittelt, bestehend aus einem breiten Spektrum von Gesunden und Kranken. Frauen wiesen in der Regel eine höhere Assoziation zu Parametern der Arteriellen Gefäßsteifigkeit auf als Männer. Bei allen Variablen der zentralen Drücke bzw. Gefäßsteifigkeitsparametern gab es signifikante Unterschiede zwischen sogenannten Hochrisiko-Gruppen und Patienten mit niedrigem Risiko. Zentrale Drücke zeigten im Vergleich zu peripheren Korrelaten eine wesentliche höhere Assoziation. Somit kamen wir zu ähnlichen Ergebnissen wie andere Arbeitsgruppen. Wir konnten dies zusätzlich für den Reynolds Risk Score und den Procam Risikoscore zeigen, zu denen bisher keine Ergebnisse publiziert sind.
Weitere Risikofaktoren
Wir untersuchten zudem die Abhängigkeit zwischen weiteren Risikofaktoren (GFR, Homozystein und hsCRP) und arterieller Gefäßsteifigkeit. Bei den Männern zeigten alle eine moderate Korrelation zu Parametern der Arteriellen Gefäßsteifigkeit. Bei Frauen war dies bei GFR und Homozystein der Fall, jedoch zeigte sich hsCRP lediglich mit CPP und CSBP signifikant assoziiert.
Fazit
Durch die Ergebnisse der vorliegenden Arbeit zeigt sich, dass auch durch oszillometrische Messungen die Parameter der Arteriellen Gefäßsteifigkeiten ermittelt werden können. Es wurden starke Assoziationen zu etablierten Risikoscores und Surrogat-Parametern nachgewiesen. Trotz in der Literatur verbliebenen Zweifel am Messprinzip scheint durch oszillometrisch gemessene Gefäßsteifigkeitsparameter ein erhöhtes kardiovaskuläres Risiko feststellbar. Es zeigten sich jedoch Grenzen der graphischen Auswertung, eine qualitative Überprüfung ist daher bei allen Messungen unerlässlich.
Background: Cardiovascular (CV) outcome trials in type 2 diabetes (T2D) have underrepresented patients with chronic kidney disease (CKD), leading to uncertainty regarding their kidney efficacy and safety. The CARMELINA (R) trial aims to evaluate the effects of linagliptin, a DPP-4 inhibitor, on both CV and kidney outcomes in a study population enriched for cardio-renal risk.
Methods: CARMELINA (R) is a randomized, double-blind, placebo-controlled clinical trial conducted in 27 countries in T2D patients at high risk of CV and/or kidney events. Participants with evidence of CKD with or without CV disease and HbA1c 6.5-10.0% (48-86 mmol/mol) were randomized 1:1 to receive linagliptin once daily or matching placebo, added to standard of care adjusted according to local guidelines. The primary outcome is time to first occurrence of CV death, non-fatal myocardial infarction, or non-fatal stroke. The key secondary outcome is a composite of time to first sustained occurrence of end-stage kidney disease, >= 40% decrease in estimated glomerular filtration rate (eGFR) from baseline, or renal death. CV and kidney events are prospectively adjudicated by independent, blinded clinical event committees. CARMELINA (R) was designed to continue until at least 611 participants had confirmed primary outcome events. Assuming a hazard ratio of 1.0, this provides 90% power to demonstrate non-inferiority of linagliptin versus placebo within the pre-specified non-inferiority margin of 1.3 at a one-sided a-level of 2.5%. If non-inferiority of linagliptin for the primary outcome is demonstrated, then its superiority for both the primary outcome and the key secondary outcome will be investigated with a sequentially rejective multiple test procedure.
Results: Between July 2013 and August 2016, 6980 patients were randomized and took >= 1 dose of study drug (40.6, 33.1, 16.9, and 9.4% from Europe, South America, North America, and Asia, respectively). At baseline, mean +/- SD age was 65.8 +/- 9.1 years, HbA1c 7.9 +/- 1.0%, BMI 31.3 +/- 5.3 kg/m(2), and eGFR 55 +/- 25 mL/min/1.73 m(2). A total of 5148 patients (73.8%) had prevalent kidney disease (defined as eGFR < 60 mL/min/1.73 m(2) or macroalbuminuria [albumin-to-creatinine ratio > 300 mg/g]) and 3990 patients (57.2%) had established CV disease with increased albuminuria; these characteristics were not mutually exclusive. Microalbuminuria (n = 2896 [41.5%]) and macroalbuminuria (n = 2691 [38.6%]) were common.
Conclusions: CARMELINA (R) will add important information regarding the CV and kidney disease clinical profile of linagliptin by including an understudied, vulnerable cohort of patients with T2D at highest cardio-renal risk.
Purpose: The metabolically most active lesion in 2-deoxy-2-(\(^{18}\)F)fluoro-D-glucose (\(^{18}\)F-FDG) PET/CT can predict progression-free survival (PFS) in patients with medullary thyroid carcinoma (MTC) starting treatment with the tyrosine kinase inhibitor (TKI) vandetanib. However, this metric failed in overall survival (OS) prediction. In the present proof of concept study, we aimed to explore the prognostic value of intratumoral textural features (TF) as well as volumetric parameters (total lesion glycolysis, TLG) derived by pre-therapeutic \(^{18}\)F-FDG PET.
Methods: Eighteen patients with progressive MTC underwent baseline \(^{18}\)F-FDG PET/CT prior to and 3 months after vandetanib initiation. By manual segmentation of the tumor burden at baseline and follow-up PET, intratumoral TF and TLG were computed. The ability of TLG, imaging-based TF, and clinical parameters (including age, tumor marker doubling times, prior therapies and RET (rearranged during transfection) mutational status) for prediction of both PFS and OS were evaluated.
Results: The TF Complexity and the volumetric parameter TLG obtained at baseline prior to TKI initiation successfully differentiated between low- and high-risk patients. Complexity allocated 10/18 patients to the high-risk group with an OS of 3.3y (vs. low-risk group, OS=5.3y, 8/18, AUC=0.78, P=0.03). Baseline TLG designated 11/18 patients to the high-risk group (OS=3.5y vs. low-risk group, OS=5y, 7/18, AUC=0.83, P=0.005). The Hazard Ratio for cancer-related death was 6.1 for Complexity (TLG, 9.5). Among investigated clinical parameters, the age at initiation of TKI treatment reached significance for PFS prediction (P=0.02, OS, n.s.).
Conclusions: The TF Complexity and the volumetric parameter TLG are both independent parameters for OS prediction.
In diabetic cardiomyopathy, left ventricular (LV) diastolic dysfunction is one of the earliest signs of cardiac involvement prior to the definitive development of heart failure (HF). We aimed to explore the LV diastolic function using electrocardiography (ECG)-gated \(^{18}\)F-fluorodeoxyglucose positron emission tomography (\(^{18}\)F-FDG PET) imaging beyond the assessment of cardiac glucose utilization in a diabetic rat model. ECG-gated \(^{18}\)F-FDG PET imaging was performed in a rat model of type 2 diabetes (ZDF fa/fa) and ZL control rats at age of 13 weeks (n=6, respectively). Under hyperinsulinemic-euglycemic clamp to enhance cardiac activity, \(^{18}\)F-FDG was administered and subsequently, list-mode imaging using a dedicated small animal PET system with ECG signal recording was performed. List-mode data were sorted and reconstructed into tomographic images of 16 frames per cardiac cycle. Left ventricular functional parameters (systolic: LV ejection fraction (EF), heart rate (HR) vs. diastolic: peak filling rate (PFR)) were obtained using an automatic ventricular edge detection software. No significant difference in systolic function could be obtained (ZL controls vs. ZDF rats: LVEF, 62.5±4.2 vs. 59.4±4.5%; HR: 331±35 vs. 309±24 bpm; n.s., respectively). On the contrary, ECG-gated PET imaging showed a mild but significant decrease of PFR in the diabetic rats (ZL controls vs. ZDF rats: 12.1±0.8 vs. 10.2±1 Enddiastolic Volume/sec, P<0.01). Investigating a diabetic rat model, ECG-gated \(^{18}\)F-FDG PET imaging detected LV diastolic dysfunction while systolic function was still preserved. This might open avenues for an early detection of HF onset in high-risk type 2 diabetes before cardiac symptoms become apparent.
Background. Fast progression of the transaortic mean gradient (P-mean) is relevant for clinical decision making of valve replacement in patients with moderate and severe aortic stenosis (AS) patients. However, there is currently little knowledge regarding the determinants affecting progression of transvalvular gradient in AS patients. Methods. This monocentric retrospective study included consecutive patients presenting with at least two transthoracic echocardiography examinations covering a time interval of one year or more between April 2006 and February 2016 and diagnosed as moderate or severe aortic stenosis at the final echocardiographic examination. Laboratory parameters, medication, and prevalence of eight known cardiac comorbidities and risk factors (hypertension, diabetes, coronary heart disease, peripheral artery occlusive disease, cerebrovascular disease, renal dysfunction, body mass index >= 30 Kg/m(2), and history of smoking) were analyzed. Patients were divided into slow (P-mean < 5 mmHg/year) or fast (P-mean >= 5 mmHg/year) progression groups. Results. A total of 402 patients (mean age 78 +/- 9.4 years, 58% males) were included in the study. Mean follow-up duration was 3.4 +/- 1.9 years. The average number of cardiac comorbidities and risk factors was 3.1 +/- 1.6. Average number of cardiac comorbidities and risk factors was higher in patients in slow progression group than in fast progression group (3.3 +/- 1.5 vs 2.9 +/- 1.7; P = 0.036). Patients in slow progression group had more often coronary heart disease (49.2% vs 33.6%; P = 0.003) compared to patients in fast progression group. LDL-cholesterol values were lower in the slow progression group (100 +/- 32.6 mg/dl vs 110.8 +/- 36.6 mg/dl; P = 0.005). Conclusion. These findings suggest that disease progression of aortic valve stenosis is faster in patients with fewer cardiac comorbidities and risk factors, especially if they do not have coronary heart disease. Further prospective studies are warranted to investigate the outcome of patients with slow versus fast progression of transvalvular gradient with regards to comorbidities and risk factors.
Catheter-related bloodstream infections (CRBSI) are major complications for patients with life-threatening conditions requiring chronic vascular catheterization. The wide range of etiologic microbes and the ongoing development of resistance to antimicrobials with specific mechanisms of action make this an appropriate target for applying a nonspecific antimicrobial therapeutic. Taurolidine hydrolyzes into two antimicrobial moieties, formaldehyde and methylene glycol, which react with microbial surfaces. NeutrolinA (R) (taurolidine, heparin, calcium citrate) was recently introduced in Germany as an antimicrobial catheter lock solution. This postmarketing experience collected data on 201 patients at 20 centers from January 2014 through September 2016. Likely CRBSI was observed in 13 episodes in 47,118 days (0.2759 per 1000 days [0.1468, 0.4718]). Thrombosed catheter was observed in seven catheters in 47,118 days (0.1486 per 1000 days [0.0595, 0.3061]). No adverse drug reactions that led to the discontinuation of NeutrolinA (R) use were reported. Two patients experienced occasional transient dysgeusia. NeutrolinA (R), when used in conjunction with guideline-based catheter care, showed reduction in the rate of both CRBSI and catheter thrombosis relative to recent historical controls.
Background
Fabry Disease (FD) is an X-linked hereditary lysosomal storage disorder which leads to a multisystemic intralysosomal accumulation of globotriaosylceramid (Gb3). Besides prominent renal and cardiac organ involvement, patients commonly complain about vestibulocochlear symptoms like high-frequency hearing loss, tinnitus and vertigo. However, comprehensive data especially on vertigo remain scarce. The aim of this study was to examine the prevalence and characteristics of vertigo and hearing loss in patients with FD, depending on renal and cardiac parameters and get hints about the site and the pattern of the lesions.
Methods
Single-center study with 57 FD patients. Every patient underwent an oto-rhino-laryngological examination as well as videonystagmography and vestibular evoked myogenic potentials (VEMPs) and audiological measurements using pure tone audiometry and auditory brainstem response audiometry (ABR). Renal function was measured by eGFR, cardiac impairment was graduated by NYHA class.
Results
More than one out of three patients (35.1%) complained about hearing loss, 54.4% about vertigo and 28.1% about both symptom. In 74% a sensorineural hearing loss of at least 25 dB was found, ABR could exclude any retrocochlear lesion. Caloric testing showed abnormal values in 71.9%, VEMPs were pathological in 68%. A correlation between the side or the shape of hearing loss and pathological vestibular testing could not be revealed.
Conclusions
Hearing loss and vertigo show a high prevalence in FD. While hearing loss seems due to a cochlear lesion, peripheral vestibular as well as central nervous pathologies cause vertigo. Thus, both the site of lesion and the pathophysiological patterns seem to differ.
Background
The role of a healthy dietary pattern in the prevention of abdominal aortic aneurysms (AAA) is unknown. We aimed to evaluate the relationship between adherence to a Dietary Approaches To Stop Hypertension‐style dietary pattern and the risk of incident AAAs.
Methods and Results
Dietary intake was assessed via a 66‐item food frequency questionnaire at baseline (1987–1989) and at visit 3 (1993–1995) in 13 496 participants enrolled in the ARIC (Atherosclerosis Risk in Communities) study without clinical AAA (mean age, 54 years). A dietary scoring index based on food times was constructed to assess self‐reported adherence to a dietary approaches to stop hypertension‐style dietary pattern. Participants were followed for incident clinical AAAs using hospital discharge diagnoses, Medicare inpatient and outpatient diagnoses, or death certificates through December 31, 2011. Cox proportional hazards models with covariate adjustment were used to estimate hazard ratios with 95% confidence intervals. During a median follow‐up of 23 years, there were 517 incident AAA cases. Individuals with a Dietary Approaches To Stop Hypertension‐style diet score in the highest quintile had a 40% lower risk of hospitalization for AAA than those in the lowest quintile (hazard ratio\(_{Q5}\) vs \(_{Q1}\): 0.60; 95% confidence intervals: 0.44, 0.83; P\(_{trend}\)=0.002). In detailed analyses, higher consumption of fruits, vegetables, whole grains, low‐fat dairy, and nuts and legumes was related to a lower risk for AAA.
Conclusions
Greater adherence to a Dietary Approaches To Stop Hypertension‐style dietary pattern was associated with lower risk for AAA. Higher consumption of fruits, vegetables, whole grains, low‐fat dairy as well as nuts and legumes may help to decrease the burden of AAAs.
Background: In end-stage renal disease (ESRD), gut-derived uremic toxins play a crucial role in the systemic inflammation and oxidative stress promoting the excess morbidity and mortality. The biochemical derangement is in part a consequence of an insufficient generation of short-chain fatty acids (SCFA) due to the dysbiosis of the gut and an insufficient consumption of the fermentable complex carbohydrates. Aim of the study: The primary end-point was to evaluate the potential efficacy of SCFA (specifically, sodium propionate (SP)) for patients on maintenance hemodialysis (MHD) on systemic inflammation. Secondary end-points included potential attenuation of oxidative stress markers, insulin resistance and production of gut-derived uremic toxins indoxyl sulfate and p-cresol sulfate, as well as health status after SP supplementation. Study design: We performed a single-center non-randomized pilot study in 20 MHD patients. They received the food additive SP with a daily intake of 2 × 500 mg in the form of capsules for 12 weeks. Pre-dialysis blood samples were taken at the beginning, after six weeks and at the end of the administration period, as well as four weeks after withdrawal of the treatment. Results: The subjects revealed a significant decline of inflammatory parameters C-reactive protein (−46%), interleukin IL-2 (−27%) and IL-17 (−15%). The inflammatory parameters IL-6 and IFN-gamma showed a mild non-significant reduction and the anti-inflammatory cytokine IL-10 increased significantly (+71%). While the concentration of bacterial endotoxins and TNF-α remained unchanged, the gut-derived uremic toxins, indoxyl sulfate (−30%) and p-cresyl sulfate (−50%), revealed a significant decline. The SP supplementation reduced the parameters of oxidative stress malondialdehyde (−32%) and glutathione peroxidase activity (−28%). The serum insulin levels dropped by 30% and the HOMA-index by 32%. The reduction of inflammatory parameters was associated with a lowering of ferritin and a significant increase in transferrin saturation (TSAT). Four weeks after the end of the treatment phase, all improved parameters deteriorated again. Evaluation of the psycho-physical performance with the short form 36 (SF-36) questionnaire showed an enhancement in the self-reported physical functioning, general health, vitality and mental health. The SP supplementation was well tolerated and without important side effects. No patient had left the study due to intolerance to the medication. The SP supplementation in MHD patients reduced pro-inflammatory parameters and oxidative stress and improved insulin resistance and iron metabolism. Furthermore, SP effectively lowered the important gut-derived uremic toxins indoxyl and p-cresol sulfate. These improvements were associated with a better quality of life. Further controlled studies are required in a larger cohort to evaluate the clinical outcome.
Technische Schwierigkeiten während der Lipidapherese beeinflussen die Fraktionatorperformance. Aus diesem Grund wurde ein Großtiermodell zur Charakterisierung neuartiger Plasmafraktionatormembranen entwickelt. Vier Schafe wurden im Rahmen einer randomisierten, kontrollierten "Cross-over"-Studie einer Doppelfiltrationsplasmapherese mit drei Varianten der neuartigen FractioPES-Membran unterzogen. Diese Varianten unterschieden sich bezüglich ihrer HDL-Siebkoeffizienten (SK) (FPESa, 0.30, FPESb, 0.26, and FPESc, 0.22). Getestet wurden diese gegen eine Kontroll-Fraktionatormembran (EVAL). Siebkoeffizienten und Reduktionsraten wurden bestimmt für LDL, HDL, Fibrinogen, IgG und Albumin. Im Vergleich zu EVAL (0.42 0.04 zu 0.74 0.08) und FPESa (0.36 0.06 zu 0.64 0.04) waren die SK für HDL niedriger (p < 0,05) von FPESc (0.30 0.04 to 0.49 0.10). Die SK für Fibrinogen waren höher mit EVAL (p < 0,05; 0.02 0.01 zu 0.40 0.08) im Vergleich zu FPESb (0.05 0.02 zu 0.26 0.34) und FPESc (0.01 0.01 to 0.21 0.16). Das Tiermodell unterschied somit die minimalen Unterschiede der Fraktionatormembranen.
Background: Pheochromocytomas and paragangliomas (PPGLs) are rare but potentially harmful tumors that can vary in their clinical presentation. Tumors may be found due to signs and symptoms, as part of a hereditary syndrome or following an imaging procedure. Objective: To investigate potential differences in clinical presentation between PPGLs discovered by imaging (iPPGLs), symptomatic cases (sPPGLs) and those diagnosed during follow-up because of earlier disease/known hereditary mutations (fPPGL). Design: Prospective study protocol, which has enrolled patients from six European centers with confirmed PPGLs. Data were analyzed from 235 patients (37 iPPGLs, 36 sPPGLs, 27% fPPGLs) and compared for tumor volume, biochemical profile, mutation status, presence of metastases and self-reported symptoms. iPPGL patients were diagnosed at a significantly higher age than fPPGLs (P<0.001), found to have larger tumors (P=0.003) and higher metanephrine and normetanephrine levels at diagnosis (P=0.021). Significantly lower than in sPPGL, there was a relevant number of self-reported symptoms in iPPGL (2.9 vs 4.3 symptoms, P< 0.001). In 16.2% of iPPGL, mutations in susceptibility genes were detected, although this proportion was lower than that in fPPGL (60.9%) and sPPGL (21.5%). Patients with PPGLs detected by imaging were older, have higher tumor volume and more excessive hormonal secretion in comparison to those found as part of a surveillance program. Presence of typical symptoms indicates that in a relevant proportion of those patients, the PPGL diagnosis had been delayed. Precis: Pheochromocytoma/paraganglioma discovered by imaging are often symptomatic and carry a significant proportion of germline mutations in susceptibility genes.
Background:
The Catechol-O-methyltransferase (COMT) represents the key enzyme in catecholamine degradation. Recent studies suggest that the COMT rs4680 polymorphism is associated with the response to endogenous and exogenous catecholamines. There are, however, conflicting data regarding the COMT Met/Met phenotype being associated with an increased risk of acute kidney injury (AKI) after cardiac surgery. The aim of the current study is to prospectively investigate the impact of the COMT rs4680 polymorphism on the incidence of AKI in patients undergoing cardiac surgery.
Methods:
In this prospective single center cohort study consecutive patients hospitalized for elective cardiac surgery including cardiopulmonary-bypass (CPB) were screened for participation. Demographic clinical data, blood, urine and tissue samples were collected at predefined time points throughout the clinical stay. AKI was defined according to recent recommendations of the Kidney Disease Improving Global Outcome (KDIGO) group. Genetic analysis was performed after patient enrolment was completed.
Results:
Between April and December 2014, 150 patients were recruited. The COMT genotypes were distributed as follows: Val/Met 48.7%, Met/Met 29.3%, Val/Val 21.3%. No significant differences were found for demography, comorbidities, or operative strategy according to the underlying COMT genotype. AKI occurred in 35 patients (23.5%) of the total cohort, and no differences were evident between the COMT genotypes (20.5% Met/Met, 24.7% Val/Met, 25.0% Val/Val, p = 0.66). There were also no differences in the post-operative period, including ICU or in-hospital stay.
Conclusions:
We did not find statistically significant variations in the risk for postoperative AKI, length of ICU or in-hospital stay according to the underlying COMT genotype.
Introduction Patients with end-stage kidney disease requiring maintenance haemodialysis treatment experience a dramatic cardiovascular morbidity and mortality. Due to the high atherosclerotic and arteriosclerotic burden and profound alterations in haemostasis, they frequently suffer and die from both thromboembolic and bleeding events. This is a particular concern in patients on haemodialysis with atrial fibrillation (AF). Controlled trials on the optimal anticoagulation in patients with AF on haemodialysis are not available. The randomised controlled phase IIIb AXADIA-AFNET 8 trial will evaluate the safety and efficacy of the factor Xa inhibitor apixaban in patients with AF requiring haemodialysis. Methods and analysis A total of 222 patients will be randomised in an open-labelled, 1:1 design to receive either apixaban 2.5mg twice daily or dose-adjusted vitamin K antagonist therapy (target international normalised ratio 2.0-3.0). All patients will be treated and followed up for a minimum of 6 months up to a maximum of 24 months. The primary outcome is major or clinically relevant, non-major bleedings or death of any cause. Secondary outcomes include stroke, cardiovascular death and other thromboembolic events, thus exploring the efficacy of apixaban. The first patient was randomised in June 2017. Ethics and dissemination The study protocol was approved by the Ethical Committee of the Landesaertzekammer, Westfalen-Lippe and the Medical Faculty of the University of Muenster, Muenster, Germany (reference number: 2016-598f-A). Written informed consent will be obtained from all patients prior to study participation, including their consent for long-term follow-up. AXADIA-AFNET 8 is an investigator-initiated trial. Sponsor is AFNET, Muenster, Germany. Study findings will be disseminated to Bristol-Myers Squibb, Munich, Germany, and Pfizer, Berlin, Germany, to the participating centres, at research conferences and in peer-reviewed journals. Background:
Mitral regurgitation in ischemic heart disease (IMR) is a strong predictor of outcome but until now, pathophysiology is not sufficiently understood and treatment is not satisfying. We aimed to systematically evaluate structural and functional mitral valve leaflet and annular characteristics in patients with IMR to determine the differences in geometric and dynamic changes of the MV between significant and mild IMR.
Methods:
Thirty-seven patients with IMR (18 mild (m)MR, 19 significant (moderate+severe) (s)MR) and 33 controls underwent TEE. 3D volumes were analyzed using 3D feature-tracking software.
Results:
All IMR patients showed a loss of mitral annular motility and non-planarity, whereas mitral annulus dilation and leaflet enlargement occurred in sMR only. Active-posterior-leaflet-area decreased in early systole in all three groups accompanied by an increase in active-anterior-leaflet-area in early systole in controls and mMR but only in late systole in sMR.
Conclusions:
In addition to a significant enlargement and loss in motility of the MV annulus, patients with significant IMR showed a spatio-temporal alteration of the mitral valve coaptation line due to a delayed increase in active-anterior-leaflet-area. This abnormality is likely to contribute to IMR severity and is worth the evaluation of becoming a parameter for clinical decision-making. Further, addressing the leaflets aiming to increase the active leaflet-area is a promising therapeutic approach for significant IMR. Additional studies with a larger sample size and post-operative assessment are warranted to further validate our findings and help understand the dynamics of the mitral valve.
The pathomechanism of primary polydipsia is poorly understood. Recent animal data reported a connection between fibroblast growth factor 21 (FGF-21) and elevated fluid intake independently of hormonal control by the hormone arginine-vasopressin (AVP) and osmotic stimulation. We therefore compared circulating FGF-21 levels in patients with primary polydipsia to patients with AVP deficiency (central diabetes insipidus) and healthy volunteers. In this prospective cohort study, we analyzed FGF-21 levels of 20 patients with primary polydipsia, 20 patients with central diabetes insipidus and 20 healthy volunteers before and after stimulation with hypertonic saline infusion targeting a plasma sodium level >= 150 mmol/L. The primary outcome was the difference in FGF-21 levels between the three groups. Baseline characteristics were similar between the groups except for patients with central diabetes insipidus being heavier. There was no difference in baseline FGF-21 levels between patients with primary polydipsia and healthy volunteers (122 pg/mL (52,277) vs 193 pg/mL (48,301), but higher levels in patients with central diabetes insipidus were observed (306 pg/mL (114,484); P=0.037). However, this was not confirmed in a multivariate linear regression analysis after adjusting for age, sex, BMI and smoking status. Osmotic stimulation did not affect FGF-21 levels in either group (difference to baseline: primary polydipsia -23 pg/mL (-43, 22); central diabetes insipidus 17 pg/mL (-76, 88); healthy volunteers -6 pg/mL (-68, 22); P=0.45). To conclude, FGF-21 levels are not increased in patients with primary polydipsia as compared to central diabetes insipidus or healthy volunteers. FGF-21 therefore does not seem to be causal of elevated fluid intake in these patients.
Increased sympathetic noradrenergic signaling is crucially involved in fear and anxiety as defensive states. MicroRNAs regulate dynamic gene expression during synaptic plasticity and genetic variation of microRNAs modulating noradrenaline transporter gene (SLC6A2) expression may thus lead to altered central and peripheral processing of fear and anxiety. In silico prediction of microRNA regulation of SLC6A2 was confirmed by luciferase reporter assays and identified hsa-miR-579-3p as a regulating microRNA. The minor (T)-allele of rs2910931 (MAFcases = 0.431, MAFcontrols = 0.368) upstream of MIR579 was associated with panic disorder in patients (pallelic = 0.004, ncases = 506, ncontrols = 506) and with higher trait anxiety in healthy individuals (pASI = 0.029, pACQ = 0.047, n = 3112). Compared to the major (A)-allele, increased promoter activity was observed in luciferase reporter assays in vitro suggesting more effective MIR579 expression and SLC6A2 repression in vivo (p = 0.041). Healthy individuals carrying at least one (T)-allele showed a brain activation pattern suggesting increased defensive responding and sympathetic noradrenergic activation in midbrain and limbic areas during the extinction of conditioned fear. Panic disorder patients carrying two (T)-alleles showed elevated heart rates in an anxiety-provoking behavioral avoidance test (F(2, 270) = 5.47, p = 0.005). Fine-tuning of noradrenaline homeostasis by a MIR579 genetic variation modulated central and peripheral sympathetic noradrenergic activation during fear processing and anxiety. This study opens new perspectives on the role of microRNAs in the etiopathogenesis of anxiety disorders, particularly their cardiovascular symptoms and comorbidities.
Der Morbus Fabry ist eine X-chromosomal rezessive, lysosomale
Speicherkrankheit, die durch eine Mutation im α - Galactosidase A Gen
verursacht wird. Dadurch werden unter anderem Bestandteile der
Plasmamembran (Globotriaosylceramide) nicht mehr degradiert und sie
akkumulieren intrazellulär. Daraus resultiert, vom anfänglichen
Einzelzellschäden, letzten Endes ein oftmals schwerer Organschaden mit
Funktionsausfällen. Die einzige kausale Therapie besteht in der Substituierung
des betroffenen Enzyms.
Der Morbus Fabry äußert sich klinisch als eine Multisystemerkrankung mit
hauptsächlich renaler, nervaler, sowie kardialer Beteiligung. Vor allem letztere
ist maßgeblich für die verkürzte Lebenserwartung verantwortlich. Die Patienten
entwickeln mit Progression der Erkrankung häufig eine linksventrikuläre
Hypertrophie, eine Herzinsuffizienz und durch die zunehmende Akkumulation
der Globotriaosylceramide entsteht im Verlauf ein fibrotischer Umbau im
Myokard. Dies ist möglicherweise auch der Entstehungsort für maligne
Rhythmusstörungen. Wissenschaftlich erforscht ist, dass supraventikuläre
sowie ventrikuläre Tachykardien bzw. Bradykardien bis hin zu Asystolie/Pausen
bei diesen Patienten auftreten können. Ebenso weiß man, dass man mit Hilfe
von so genannten Event Recordern, die kontinuierlich die elektrische
Herzaktivität überwachen und die Daten via Telemetrie an ein Zentrum senden,
die Detektionsrate von Rhythmusstörungen erhöhen kann.
Aber ob solch ein Event Recorder auch bei Patienten mit fortgeschrittener
Fabry - Kardiomyopathie einen Nutzen hat und sie bei diesen Patienten zur
Detektion von malignen Rhythmusstörungen beitragen ist bisher unklar und
Thema dieser Studie.

gesicherten Morbus Fabry, einen Event Recorder. Sie erhielten 7,4 ± 4,5
Jahren die Enzymersatztherapie, wurden über einen Zeitraum von 0,3 - 2
Jahren beobachtet und übertrugen ihre Daten durchschnittlich 14 ± 11 mal pro
Monat. Dabei konnten insgesamt 8547 klinisch relevante Übertragungen aufgezeichnet werden, die entsprechend der Studieneinteilung in Asystolie,
Bradykardie, Vorhofflimmern, und ventrikuläre Tachykardie eingeteilt worden
sind.
Asystolie Episoden, mit elektrischen Pausen von 3,3 bis 4,4 Sekunden, wurden
insgesamt 66-mal bei 4 Patienten mit dem Event Recorder aufgezeichnet.
Über 8000 Bradykardien konnten bei 6 Männern und 1 Frau dokumentiert
werden, darunter ein AV-Block II° Typ Mobitz mit ei ner 2:1 Überleitung.
Fast 370-mal konnte ein intermittierendes Vorhofflimmern bzw. Vorhofflattern,
mit Flimmerzeiten von 10 Sekunden bis maximal 86400 Sekunden, dargestellt
werden. Bei insgesamt 5 Patienten konnten 10 ventrikuläre Tachykardie –
Episoden, mit einer maximalen Herzfrequenz 206 Schlägen / min, durch den
Event Recorder aufgezeichnet werden.
So konnten selbst bei dieser kleinen Kohorte, mit dem Event Recorder, viele
klinisch relevante Herzrhythmusstörungen detektiert werden. Auf Grundlage
dieser Daten sprach man im Verlauf bei den entsprechenden Patienten eine
Empfehlung zur Therapieänderungen aus um klinische Komplikationen zu
verhindern.
Dies führte letzten Endes zu der Schlussfolgerung, dass der Einsatz von Event
Recordern sicherlich ein sehr nützliches diagnostisches Instrument zur
Detektion von malignen Rhythmusstörungen bei Patienten mit einer
fortgeschrittenen Fabry-Kardiomyopathie ist.
Es sollte nun weiter geprüft werden, ob der Event Recorder bereits in früheren
Stadien des Morbus Fabry zum Einsatz kommen sollte.
Einleitung: Die linksventrikuläre diastolische Dysfunktion (LVDD) ist bei Diabetikern noch vor Entwicklung einer klinisch apparenten Herzinsuffizienz eines der ersten Anzeichen einer kardialen Beteiligung. Daher soll in dieser Studie untersucht werden, ob die LVDD mit ECG-gated F-18-FDG PET in einem Diabetes-Rattenmodell dargestellt werden kann.
Methodik: Es wurden F-18-FDG PET Scans in einem Typ-2-Diabetes Rattenmodell (ZDF fa/fa, n=6) und in ZL Kontrollen (n=6) vorgenommen (Alter, jeweils 13 Wochen). Unter Hyperinsulinemic-Euglycemic Clamp-Technik wurden 37 MBq 18F-FDG über die Schwanzvene appliziert. 15-35 Minuten nach Tracergabe wurden mittels eines Kleintier-PET-Scanners sowie unter EKG-Ableitung PET Scans angefertigt (16 frames/cardiac cycle). Die linksventrikuläre Ejektionsfraktion (EF) und die Peak Füllrate (PFR) wurden mittels einer geeigneten Software (Heart Function View) gemessen, wobei die Software an die Größe des Rattenherzes angepasst wurde.
Ergebnisse: Im Alter von 13 Wochen entwickeln ZDF Diabetes-Ratten eine im Vergleich zu Kontrolltieren eine signifikante myokardiale Hypertrophie, bestätigt durch post-mortem Analyse des Herzgewichtes (994±78mg vs. 871±44mg in ZDF Diabetes-Ratten vs. ZL Kontrollen, p<0.01). ECG-gated PET zeigte eine signifikante Abnahme der LV diastolischen PFR (10.4±0.5 vs. 11.8±0.4 EDV/sec in ZDF Diabetes-Ratten vs. ZL Kontrollen, p<0.001), jedoch zeigte sich kein signifikanter Unterschied zwischen LVEF und der Herzfrequenz in den untersuchten ZDF Diabetes-Ratten und Kontrollen (LVEF: 60.0±4.5 vs. 63.7±4.1%, n.s. und HR: 305±25 vs. 323±24 bpm, n.s.).
Schlussfolgerung: Im Diabetes-Ratten-Modell kann unter Verwendung eines ECG-gated FDG-PET Protokolls die diastolische Dysfunktion als Parameter der frühen diabetischen Kardiomyopathie nachgewiesen werden.
Die Erkrankung an einem Nebennierenkarzinom ist bis heute trotz der vielfältigen Therapieansätze mit einer sehr schlechten Prognose verbunden. Die Entwicklung von [131I]Iodmetomidat und dessen Anwendung bei Patienten im metastasierten Tumorstadium zeigte großes therapeutisches Potenzial. Aufgrund des enzymatischen Abbaus ist die Verweildauer und effektive Dosis im Tumorgewebe jedoch reduziert, sodass in dieser Arbeit nach einer metabolisch stabileren Substanz bei hoher Affinität zum Zielgewebe und gleichzeitig reduzierter Hintergrundaktivität gesucht wurde. Es wurden mehr als 80 IMTO-Derivate synthetisiert und anschließend deren metabolische Stabilität nach Inkubation mit hepatischen Esterasen mittels Radio-HPLC analysiert. Für die Substanzen [125I]IMTO-Azetidinylamid und [125I]IMTO-Ethylmethylamid wurden aufgrund ihrer mit [125I]Iodmetomidat vergleichbaren chemisch-physikalischen Eigenschaften beziehungsweise ihrer besseren metabolischen Stabilität in vitro-Zellversuche zur Evaluation der Aufnahme der Substanzen in NCI H295-Zellen durchgeführt. Hierbei ergab sich kein signifikanter Unterschied bezüglich einer Aufnahme von [125I]IMTO-Azetidinylamid. [125I]IMTO-Ethylmethylamid wurde signifikant schlechter aufgenommen. Ein Uptake-Versuch mit [125I]IMTO-Azetidinylamid unter zeitgleicher Inkubation mit nicht-radioaktiv markiertem Etomidat ergab Hinweise auf einen kompetitiven Aufnahmemechanismus analog der Referenzsubstanz [125I]Iodmetomidat.
Mittels Mitochondrien-Isolationsversuchen festigten sich Hinweise auf eine dem [125I]Iodmetomidat ähnliche Aufnahme der Substanz [125I]IMTO-Azetidinylamid in mitochondriale Strukturen der NCI H295-Zellen.
Zur Evaluation des Verhaltens der Substanzen [125I] Iodmetomidat, [125I]IMTO-Azetidinylamid und [125I]IMTO-Ethylmethylamid wurden in vivo-Versuche an männlichen CD1-Mäusen durchgeführt.
Hierbei ergaben sich nach intravenöser Injektion und Messung der relativen Organdosen nach definierten Zeitintervallen deutlich höhere und längere Anreicherungen der Substanz [125I]IMTO-Azetidinylamid im Nebennierengewebe bei gleichzeitig sowohl initial als auch im Verlauf deutlich reduzierter Restorgandosis im Vergleich zur Referenz.
Eine extern durchgeführte Toxizitätsstudie ergab Hinweise auf dosisabhängige klinische Effekte, welche im Vergleich zu Etomidat jedoch deutlich geringer ausfielen. Insgesamt gab es weder Hinweise auf eine erhöhte Mortalität oder einen hämatologischen Effekt noch auf biochemische oder pathologische Veränderungen nach Applikation der Substanz IMTO-Azetidinylamid. Ein in Auftrag gegebener Ames-Test ergab keinen Hinweis auf eine mögliche Mutagenität der Substanz.
Die nach Abschluss des experimentellen Teils dieser Arbeit durch die Arbeitsgruppe um Prof. Dr. Stefanie Hahner und Dr. Andreas Schirbel durchgeführte Anwendung der Substanz [123/131I]IMTO-Azetidinylamid erbrachte vielversprechende Ergebnisse. Im Vergleich zu [125I]Iodmetomidat zeigte [125I]IMTO-Azetidinylamid aufgrund seiner hochspezifischen Aufnahme in das Zielgewebe ein deutlich besseres Profil bezüglich eines diagnostischen und therapeutischen Einsatzes.
Das Iodmetomidat-Carbonsäureamid [123/131I]IMTO-Azetidinylamid ist ein vielversprechender, im Vergleich zu [123/131I]IMTO metabolisch stabilerer Radiotracer zur Diagnostik adrenaler Läsionen und könnte bei gleichzeitig reduzierten radiotoxischen Nebenwirkungen zur Verbesserung der Therapie adrenaler Karzinome beitragen.
Bezüglich einer generellen Empfehlung der Anwendung von [123/131I]IMTO-Azetidinylamid in der Diagnostik und Therapie von Nebennierenkarzinomen sollten zunächst weitere Untersuchungen durchgeführt werden.
Die vorliegende Dissertation untersucht den Zusammenhang zwischen der Ausprägung physischer Aktivität und der Prävalenz kardiovaskulärer Risikofaktoren und Komorbiditäten bei Patienten mit milder bis moderater chronischer Niereninsuffizienz (CKD). Ziel ist die Identifikation kardiovaskulärer Risikofaktoren und Komorbiditäten, die durch physische Aktivität bei diesem Patientenkollektiv positiv beeinflusst werden können.
Grundlagen: Es besteht eine deutlich gesteigerte kardiovaskuläre Morbidität und Mortalität für Patienten mit CKD. Diese versterben häufig bereits aufgrund kardiovaskulärer Ereignisse bevor sie das Stadium des ESRD erreichen. Besonders den "traditionellen" kardiovaskulären Risikofaktoren, deren Prävalenz bei CKD deutlich gesteigert ist, kommt im Zusammenhang zwischen CKD und CVD eine bedeutende Rolle zu. Im Rahmen dieser Studie wurden unter den "traditionellen" Risikofaktoren diejenigen zusammengefasst, deren Gesamtheit das metabolische Syndrom definiert. Es gilt als belegt, dass physische Aktivität bei gesunden Personen einen präventiven Einfluss auf diese "traditionellen" Risikofaktoren einer CVD besitzt. In wieweit dies auf CKD Patienten mit ihrer vielfältigen weiteren kardiovaskulären Risikofaktoren übertragen werden darf und welchen Einfluss physische Aktivität hier haben kann, ist ungewiss. Es besteht allerdings Evidenz für eine deutliche Reduktion der physischen Aktivität bei Vorliegen einer milden bis moderaten CKD.
Methodik: Die prospektive klinisch-experimentelle Pedometerstudie stellt die erste objektiv erhobene Datensammlungen physischer Aktivität dieser Größe von Patienten mit CKD im Stadium 3 dar. Die 1153 eingeschlossenen Probanden wurden bundesweit aus der multizentrischen GCKD-Studie rekrutiert. Die Schrittzahlerfassung wurde über einen Zeitraum von 13 konsekutiven Tagen durchgeführt. Unter Berücksichtigung standardisiert erhobener Laborwerte und Fragebögen wurde die Assoziation physischer Aktivität mit kardiovaskulären Risikofaktoren und Komorbiditäten ermittelt.
Ergebnisse: Die physische Aktivität der chronisch nierenerkrankten Studienpopulation erweist sich objektiv gemessen als deutlich eingeschränkt. Mit sinkendem Aktivitätsniveau geht eine gesteigerte kardiovaskuläre Morbidität einher. Für die Faktoren HbA1c, Alter und WHR ist eine inverse Korrelation zur Schrittzahl gegeben. Es konnte gezeigt werden, dass die mittlere Schrittzahl von Probanden, die an den kardiovaskulären Komorbiditäten pAVK, KHK, Herzinsuffizienz und Schlaganfall leiden oder die Risikofaktoren Diabetes mellitus und Adipositas aufweisen, hochsignifikant geringer ist als diejenige Schrittzahl von Probanden, die diese Faktoren nicht aufweisen. Darüber hinaus zeigt die durchgeführte Regressionsanalyse, dass auch nach Adjustierung für Alter, Geschlecht, Bildungsstatus und Anstellung mit einer höheren Schrittzahl ein signifikant gemindertes Risiko bezüglich des Vorliegens der Variablen pAVK, KHK, Herzinsuffizienz, Diabetes mellitus, Adipositas und Rauchen gegeben ist als in einer weniger aktiven Vergleichsgruppe mit der gleichen Nierenfunktionseinschränkung. Bezüglich der kardiovaskulären Risikofaktoren ist somit festzustellen, dass vor allem die Risikofaktoren Adipositas und Diabetes mellitus eine Beeinflussbarkeit aufgrund des Faktors physische Aktivität zeigen.
Fazit: Eine reduzierte physische Aktivität ist auch bei Patienten mit CKD mit einem höheren kardiovaskulären Risiko verbunden. Bei Patienten mit CKD können durch physische Aktivität vor allem die kardiovaskulären Risikofaktoren Adipositas und Diabetes mellitus positiv beeinflusst werden. Die krankheitsbedingt deutlich beeinflussten Risikofaktoren arterielle Hypertonie und Dyslipidämie unterliegen bei dieser Studienpopulation hingegen keinem messbaren positiven Effekt physischer Aktivität. Daher sollte insbesondere Patienten mit chronischem Nierenleiden, die zudem an einer Adipositas und oder einem Diabetes mellitus leiden, zu physischer Aktivität geraten werden.
Brown adipose tissue (BAT) is an attractive therapeutic target to combat diabetes and obesity due to its ability to increase glucose expenditure. In a genetic rat model (ZDF fa/fa) of type-2 diabetes and obesity, we aimed to investigate glucose utilization of BAT by \(^{18}\)F-FDG PET imaging. Male Zucker diabetic fatty (ZDF) and Male Zucker lean (ZL) control rats were studied at 13 weeks. Three weeks prior to imaging, ZDF rats were randomized into a no-restriction (ZDF-ND) and a mild calorie restriction (ZDF-CR) group. Dynamic \(^{18}\)F-FDG PET using a dedicated small animal PET system was performed under hyperinsulinemic-euglycemic clamp. \(^{18}\)F-FDG PET identified intense inter-scapular BAT glucose uptake in all ZL control rats, while no focally increased \(^{18}\)F-FDG uptake was detected in all ZDF-ND rats. Mild but significant improved BAT tracer uptake was identified after calorie restriction in diabetic rats (ZDF-CR). The weight of BAT tissue and fat deposits were significantly increased in ZDF-CR and ZDF-ND rats as compared to ZL controls, while UCP-1 and mitochondrial concentrations were significantly decreased. Whitening and severely impaired insulin-stimulated glucose uptake in BAT was confirmed in a rat model of type-2 diabetes. Additionally, calorie restriction partially restored the impaired BAT glucose uptake.
Introduction: The prognosis of medullary thyroid carcinoma (MTC) is poor using common chemotherapeutic approaches. However, during the last years encouraging results of recently introduced tyrosine kinase inhibitors (TKI) such as vandetanib have been published. In this study we aimed to correlate the results of \(^{18}\)F-fluorodeoxyglucose ([\(^{18}\)F]FDG) positron emission tomography (PET) imaging with treatment outcome.
Methods: Eighteen patients after thyroidectomy with recurrent/advanced MTC lesions receiving vandetanib (300 mg orally/day) could be analysed. A baseline \(^{18}\)F-FDG PET prior to and a follow-up \(^{18}\)F-FDG PET 3 months after TKI initiation were performed. During follow-up, tumor progression was assessed every 3 months including computed tomography according to RECIST. Progression-free survival (PFS) was correlated with the maximum standardized uptake value of \(^{18}\)F-FDG in lymph nodes (SUV(LN)max) or visceral metastases (SUV(MTS)max) as well as with clinical parameters using ROC analysis.
Results: Within median 3.6 years of follow-up, 9 patients showed disease progression at median 8.5 months after TKI initiation. An elevated glucose consumption assessed by baseline \(^{18}\)F-FDG PET (SUV(LN)max > 7.25) could predict a shorter PFS (2 y) with an accuracy of 76.5% (SUV(LN)max <7.25, 4.3 y; p=0.03). Accordingly, preserved tumor metabolism in the follow-up PET (SUV(MTS)max >2.7) also demonstrated an unfavorable prognosis (accuracy, 85.7%). On the other hand, none of the clinical parameters reached significance in response prediction.
Conclusions: In patients with advanced and progressive MTC, tumors with higher metabolic activity at baseline are more aggressive and more prone to progression as reflected by a shorter PFS; they should be monitored more closely. Preserved glucose consumption 3 months after treatment initiation was also related to poorer prognosis.
C-X-C motif chemokine receptor 4 (CXCR4) is a key factor for tumor growth and metastasis in several types of human cancer. This study investigated the feasibility of CXCR4-directed imaging with positron emission tomography/computed tomography (PET/CT) using [\(^{68}\)Ga]Pentixafor in malignant pleural mesothelioma.
Six patients with pleural mesothelioma underwent [\(^{68}\)Ga]Pentixafor-PET/CT. 2′-[\(^{18}\)F]fluoro-2′-deoxy-D-glucose ([\(^{18}\)F]FDG)-PET/CT (4/6 patients) and immunohistochemistry obtained from biopsy or surgery (all) served as standards of reference. Additionally, 9 surgical mesothelioma samples were available for histological work-up.
Whereas [\(^{18}\)F]FDG-PET depicted active lesions in all patients, [\(^{68}\)Ga]Pentixafor-PET/CT recorded physiologic tracer distribution and none of the 6 patients presented [\(^{68}\)Ga]Pentixafor-positive lesions. This finding paralleled results of immunohistochemistry which also could not identify relevant CXCR4 surface expression in the samples analyzed.
In contrast to past reports, our data suggest widely absence of CXCR4 expression in pleural mesothelioma. Hence, robust cell surface expression should be confirmed prior to targeting this chemokine receptor for diagnosis and/or therapy.
Livin/BIRC7 is a member of the inhibitors of apoptosis proteins family, which are involved in tumor development through the inhibition of caspases. Aim was to investigate the expression of livin and other members of its pathway in adrenocortical tumors and in the adrenocortical carcinoma (ACC) cell line NCI-H295R.
The mRNA expression of livin, its isoforms α and β, XIAP, CASP3 and DIABLO was evaluated by qRT-PCR in 82 fresh-frozen adrenal tissues (34 ACC, 25 adenomas = ACA, 23 normal adrenal glands = NAG). Livin protein expression was assessed by immunohistochemistry in 270 paraffin-embedded tissues (192 ACC, 58 ACA, 20 NAG). Livin, CASP3 and cleaved caspase-3 were evaluated in NCI-H295R after induction of livin overexpression.
Relative livin mRNA expression was significantly higher in ACC than in ACA and NAG (0.060 ± 0.116 vs 0.004 ± 0.014 and 0.002 ± 0.009, respectively, p < 0.01), being consistently higher in tumors than in adjacent NAG and isoform β more expressed than α. No significant differences in CASP3, XIAP and DIABLO levels were found among these groups. In immunohistochemistry, livin was localized in both cytoplasm and nuclei. The ratio between cytoplasmic and nuclear staining was significantly higher in ACC (1.51 ± 0.66) than in ACA (0.80 ± 0.35) and NAG (0.88 ± 0.27; p < 0.0001). No significant correlations were observed between livin expression and histopathological parameters or clinical outcome. In NCI-H295R cells, the livin overexpression slightly reduced the activation of CASP3, but did not correlate with cell viability.
In conclusion, livin is specifically over-expressed in ACC, suggesting that it might be involved in adrenocortical tumorigenesis and represent a new molecular marker of malignancy.
Zusammenfassend lässt sich festhalten, dass in dieser Arbeit 15 neu entwickelte Substanzen zur selektiven und hochaffinen Blockade der Aldosteronsynthase untersucht werden konnten. Es wurden mehrere neue aufeinander aufbauende Testsysteme etabliert, um die neuen Substanzen auf ihre Selektivität und Affinität gegenüber der Aldosteronsynthase zu untersuchen. Eine Testung der Inhibition der humanen Aldosteronsynthase und der 11β-Hydroxylase zuerst in getrennten Zellkulturansätzen, die die humanen Enzyme stabil exprimieren, und anschließend in der NCI-h295 Zelllinie, die beide Enzyme und zusätzlich die meisten anderen Enzyme der Steroidbiosynthese stabil exprimieren, ist eine gute Voraussetzung, um selektive und hochaffine Aldosteronsynthaseinhibitoren zu finden. Hier konnten sechs Inhibitoren ausgewählt werden, die hochaffin und selektiv an die Aldosteronsynthase binden und diese inhibieren. Die weitere Testung der [18F] markierten Substanzen zeigte für eine Substanz eine hochaffine und selektive Bindung an humanes adrenales Gewebe und keine unspezifische Bindung an andere humane Gewebe. Hier liegt die Voraussetzung vor, den Tracer weiteren in vivo Studien zuzuführen, um am humanisierten Mausmodell zu untersuchen, ob eine Bindung in vivo entsprechend den vielversprechenden Ergebnissen in vitro abläuft. Auch die ex vivo Studie an Nebennieren einer gegenüber der CYP11B2 humanisierten Maus bekräftigte diese Ergebnisse. Mit Hilfe dieser Untersuchungsmethoden lassen sich in Zukunft noch weiter entwickelte Substanzen umfangreich auf ihre Selektivität, Spezifität und Affinität testen. Dies dient als Grundlage für weitere Untersuchungen zur Entwicklung eines PET-Tracers für die Differentialdiagnostik bei primärem Hyperaldosteronismus. Eine Erkrankung, die häufiger ist als vermutet, und bei der die Differentialdiagnostik die entscheidende Voraussetzung für die Einleitung einer Therapie ist, die sich entweder operativ oder medikamentös darstellt. Bisherige differentialdiagnostische Vorgehensweisen beim primären Hyperaldosteronismus bieten aktuell keine zufriedenstellenden Ergebnisse; dies kann sich mit der Einführung eines neuen PET Tracers ändern.
Background:
Adherence to pharmacotherapeutic treatment guidelines in patients with heart failure (HF) is of major prognostic importance, but thorough implementation of guidelines in routine care remains insufficient. Our aim was to investigate prevalence and characteristics of HF in patients with coronary heart disease (CHD), and to assess the adherence to current HF guidelines in patients with HF stage C, thus identifying potential targets for the optimization of guideline implementation.
Methods:
Patients from the German sample of the European Action on Secondary and Primary Prevention by Intervention to Reduce Events (EuroAspire) IV survey with a hospitalization for CHD within the previous six to 36 months providing valid data on echocardiography as well as on signs and symptoms of HF were categorized into stages of HF: A, prevalence of risk factors for developing HF; B, asymptomatic but with structural heart disease; C, symptomatic HF. A Guideline Adherence Indicator (GAI-3) was calculated for patients with reduced (≤40%) left ventricular ejection fraction (HFrEF) as number of drugs taken per number of drugs indicated; beta-blockers, angiotensin converting enzyme inhibitors/angiotensin receptor blockers, and mineralocorticoid receptor antagonists (MRA) were considered.
Results:
509/536 patients entered analysis. HF stage A was prevalent in n = 20 (3.9%), stage B in n = 264 (51.9%), and stage C in n = 225 (44.2%) patients; 94/225 patients were diagnosed with HFrEF (42%). Stage C patients were older, had a longer duration of CHD, and a higher prevalence of arterial hypertension. Awareness of pre-diagnosed HF was low (19%). Overall GAI-3 of HFrEF patients was 96.4% with a trend towards lower GAI-3 in patients with lower LVEF due to less thorough MRA prescription.
Conclusions:
In our sample of CHD patients, prevalence of HF stage C was high and a sizable subgroup suffered from HFrEF. Overall, pharmacotherapy was fairly well implemented in HFrEF patients, although somewhat worse in patients with more reduced ejection fraction. Two major targets were identified possibly suited to further improve the implementation of HF guidelines: 1) increase patients´ awareness of diagnosis and importance of HF; and 2) disseminate knowledge about the importance of appropriately implementing the use of mineralocorticoid receptor antagonists.

This is a cross-sectional analysis of a non-interventional study. Therefore, it was not registered as an interventional trial.
Somatic mutations in protein kinase A catalytic α subunit (PRKACA) were found to be causative for 30-40% of cortisol-producing adenomas (CPA) of the adrenal gland, rendering PKA signalling constitutively active. In its resting state, PKA is a stable and inactive heterotetramer, consisting of two catalytic and two regulatory subunits with the latter inhibiting PKA activity. The human genome encodes three different PKA catalytic subunits and four different regulatory subunits that are preferentially expressed in different organs. In normal adrenal glands all regulatory subunits are expressed, while CPA exhibit reduced protein levels of the regulatory subunit IIβ. In this study, we linked for the first time the loss of RIIβ protein levels to the PRKACA mutation status and found the down-regulation of RIIβ to arise post-transcriptionally. We further found the PKA subunit expression pattern of different tumours is also present in the zones of the normal adrenal cortex and demonstrate that the different PKA subunits have a differential expression pattern in each zone of the normal adrenal gland, indicating potential specific roles of these subunits in the regulation of different hormones secretion.
Introduction: Calciphylaxis/calcific uremic arteriolopathy affects mainly end-stage kidney disease patients but is also associated with malignant disorders such as myeloma, melanoma and breast cancer. Genetic risk factors of calciphylaxis have never been studied before.
Methods: We investigated 10 target genes using a tagging SNP approach: the genes encoding CD73/ ecto-5'-nucleotidase (purinergic pathway), Matrix Gla protein, Fetuin A, Bone Gla protein, VKORC1 (all related to intrinsic calcification inhibition), calcium-sensing receptor, FGF23, Klotho, vitamin D receptor, stanniocalcin 1 (all related to CKD-MBD). 144 dialysis patients from the German calciphylaxis registry were compared with 370 dialysis patients without history of CUA. Genotyping was performed using iPLEX Gold MassARRAY(Sequenom, San Diego, USA), KASP genotyping chemistry (LGC, Teddington, Middlesex, UK) or sequencing. Statistical analysis comprised logistic regression analysis with adjustment for age and sex.
Results: 165 SNPs were finally analyzed and 6 SNPs were associated with higher probability for calciphylaxis (OR>1) in our cohort. Nine SNPs of three genes (CD73, FGF23 and Vitamin D receptor) reached nominal significance (p< 0.05), but did not reach statistical significance after correction for multiple testing. Of the CD73 gene, rs4431401 (OR = 1.71, 95%CI 1.08-2.17, p = 0.023) and rs9444348 (OR = 1.48, 95% CI 1.11-1.97, p = 0.008) were associated with a higher probability for CUA. Of the FGF23 and VDR genes, rs7310492, rs11063118, rs13312747 and rs17882106 were associated with a higher probability for CUA.
Conclusion: Polymorphisms in the genes encoding CD73, vitamin D receptor and FGF23 may play a role in calciphylaxis development. Although our study is the largest genetic study on calciphylaxis, it is limited by the low sample sizes. It therefore requires replication in other cohorts if available.
Increased aortic stiffness is known to be associated with atherosclerosis and has a predictive value for cardiovascular events. This study aims to investigate the local distribution of early arterial stiffening due to initial atherosclerotic lesions. Therefore, global and local pulse wave velocity (PWV) were measured in ApoE\(^{-/-}\) and wild type (WT) mice using ultrahigh field MRI. For quantification of global aortic stiffness, a new multi-point transit-time (TT) method was implemented and validated to determine the global PWV in the murine aorta. Local aortic stiffness was measured by assessing the local PWV in the upper abdominal aorta, using the flow/area (QA) method. Significant differences between age matched ApoE\(^{-/-}\) and WT mice were determined for global and local PWV measurements (global PWV: ApoE\(^{-/-}\): 2.7 ±0.2m/s vs WT: 2.1±0.2m/s, P<0.03; local PWV: ApoE\(^{-/-}\): 2.9±0.2m/s vs WT: 2.2±0.2m/s, P<0.03). Within the WT mouse group, the global PWV correlated well with the local PWV in the upper abdominal aorta (R\(^2\) = 0.75, P<0.01), implying a widely uniform arterial elasticity.
In ApoE\(^{-/-}\) animals, however, no significant correlation between individual local and global PWV was present (R\(^2\) = 0.07, P = 0.53), implying a heterogeneous distribution of vascular stiffening in early atherosclerosis. The assessment of global PWV using the new multi-point TT measurement technique was validated against a pressure wire measurement in a vessel
phantom and showed excellent agreement. The experimental results demonstrate that vascular stiffening caused by early atherosclerosis is unequally distributed over the length of large vessels. This finding implies that assessing heterogeneity of arterial stiffness by multiple local measurements of PWV might be more sensitive than global PWV to identify early atherosclerotic lesions.
Context: Patients with primary adrenal insufficiency (PAI) or congenital adrenal hyperplasia (CAH) are at a high risk of adrenal crisis (AC). Glucocorticoid sensitivity is at least partially genetically determined by polymorphisms of the glucocorticoid receptor (GR).
Objectives: To determine if a number of intercurrent illnesses and AC are associated with the GR gene polymorphism \(Bcl\)I in patients with PAI and CAH.
Design and patients: This prospective, longitudinal study over 37.7 ± 10.1 months included 47 PAI and 25 CAH patients. During the study period, intercurrent illness episodes and AC were documented.
Results: The study period covered 223 patient years in which 21 AC occurred (9.4 AC/100 pat years). There were no significant differences between \(Bcl\)I polymorphisms (CC (n=29), CG (n=34) and GG (n=9)) regarding BMI, hydrocortisone equivalent daily dose and blood pressure. We did not find a difference in the number of intercurrent illnesses/patient year among \(Bcl\)I polymorphisms (CC (1.5±1.4/pat year), CG (1.2±1.2/pat year) and GG (1.6±2.2/pat year)). The occurrence of AC was not significantly different among the homozygous (GG) genotype (32.5 AC/100 pat years), the CC genotype (6.7 AC/100 pat years) and the CG genotype (4.9 AC/100 pat years). Concomitant hypothyroidism was the highest in the GG genotype group (5/9), compared to others (CC (11/29) and CG (11/34)).
Conclusions: Although sample sizes were relatively small and results should be interpreted with caution, this study suggests that the GR gene polymorphism \(Bcl\)I may not be associated with the frequencies of intercurrent illnesses and AC.
Background
Chronic kidney disease (CKD) is a common comorbid condition in coronary heart disease (CHD). CKD predisposes the patient to acute kidney injury (AKI) during hospitalization. Data on awareness of kidney dysfunction among CHD patients and their treating physicians are lacking. In the current cross-sectional analysis of the German EUROASPIRE IV sample we aimed to investigate the physician’s awareness of kidney disease of patients hospitalized for CHD and also the patient’s awareness of CKD in a study visit following hospital discharge.
Methods
All serum creatinine (SCr) values measured during the hospital stay were used to describe impaired kidney function (eGFR\(_{CKD-EPI}\) < 60 ml/min/1.73m2) at admission, discharge and episodes of AKI (KDIGO definition). Information extracted from hospital discharge letters and correct ICD coding for kidney disease was studied as a surrogate of physician’s awareness of kidney disease. All patients were interrogated 0.5 to 3 years after hospital discharge, whether they had ever been told about kidney disease by a physician.
Results
Of the 536 patients, 32% had evidence for acute or chronic kidney disease during the index hospital stay. Either condition was mentioned in the discharge letter in 22%, and 72% were correctly coded according to ICD-10. At the study visit in the outpatient setting 35% had impaired kidney function. Of 158 patients with kidney disease, 54 (34%) were aware of CKD. Determinants of patient’s awareness were severity of CKD (OR\(_{eGFR}\) 0.94; 95%CI 0.92–0.96), obesity (OR 1.97; 1.07–3.64), history of heart failure (OR 1.99; 1.00–3.97), and mentioning of kidney disease in the index event’s hospital discharge letter (OR 5.51; 2.35–12.9).
Conclusions
Although CKD is frequent in CHD, only one third of patients is aware of this condition. Patient’s awareness was associated with kidney disease being mentioned in the hospital discharge letter. Future studies should examine how raising physician’s awareness for kidney dysfunction may improve patient’s awareness of CKD.
Background
Depression is a common comorbidity in patients with chronic heart failure (HF) and linked to a wider range of symptoms which, in turn, are linked to a decreased health-related quality of life (HRQOL). Treatment of depression might improve HRQOL but detecting depression is difficult due to the symptom overlap between HF and depression. Therefore, clinical guidelines recommend to routinely screen for depression in HF patients. No studies have so far investigated the treatment after getting aware of a depressive symptomatology and its correlation with HRQOL in primary care HF patients. Therefore, we examined the factors linked to depression treatment and those linked to HRQOL in HF patients. We hypothesized that GPs’ awareness of depressive symptomatology was associated with depression treatment and HRQOL in HF patients.
Methods
For this observational study, HF patients were recruited in primary care practices and filled out a questionnaire including PHQ-9 and HADS. A total of 574 patients screened positive for depressive symptomatology. Their GPs were interviewed by phone regarding the patients’ comorbidities and potential depression treatment. Descriptive and regression analysis were performed.
Results
GPs reported various types of depression treatments (including dialogue/counselling by the GP him/herself in 31.8% of the patients). The reported rates differed considerably between GP-reported initiated treatment and patient-reported utilised treatment regarding psychotherapy (16.4% vs. 9.5%) and pharmacotherapy (61.2% vs. 30.3%). The GPs' awareness of depressive symptomatology was significantly associated with the likelihood of receiving pharmacotherapy (OR 2.8; p < 0.001) but not psychotherapy. The patient’s HRQOL was not significantly associated with the GPs' awareness of depression.
Conclusion
GPs should be aware of the gap between GP-initiated and patient-utilised depression treatments in patients with chronic HF, which might lead to an undersupply of depression treatment. It remains to be investigated why GPs’ awareness of depressive symptomatology is not linked to patients’ HRQOL. We hypothesize that GPs are aware of cases with reduced HRQOL (which improves under depression treatment) and unaware of cases whose depression do not significantly impair HRQOL, resulting in comparable levels of HRQOL in both groups. This hypothesis needs to be further investigated.
Herzinsuffizienz, Depression und Angststörungen treten gehäuft gemeinsam auf und beeinflussen teilweise gegenseitig ihre Prognose. Die Zusammenhänge zwischen diesen Erkrankungen sind bislang nicht aufgeklärt.
In der vorliegenden Arbeit führte ischämische Herzinsuffizienz im Mausmodell zu Depressions-ähnlichem Verhalten innerhalb von 8 Wochen nach Infarktinduktion. Weiter zeigte sich eine Minderung der Gedächtnisleistung. Angst-assoziiertes Verhalten ließ sich nicht nachweisen. Immunhistochemisch konnten keine Veränderungen in spezifischen Hirnarealen nachgewiesen werden. Molekulare Methoden legen Veränderungen des Serotoninstoffwechsels als mögliche Erklärung nahe.
Nach operativer Ligatur eines Herzkrankgefäßes wurden C57/Bl6N Mäuse über einen Zeitraum von 8 Wochen beobachtet. In dieser Zeit wurden neben Herzultraschalluntersuchungen eine Reihe von Verhaltenstest durchgeführt, um depressive und ängstliche Verhaltensstrukturen sowie die kognitive Leistungsfähigkeit beurteilen zu können. Nach Ablauf des Beobachtungszeitraumes wurden das Herz und das Gehirn entnommen und weiteren histologischen und molekularen Untersuchungen zugeführt.
Die histologische Aufarbeitung des Herzens nach Ende des Versuchszeitraumes bestätigte die Beobachtungen anderen Autoren, dass eine Infarktgröße von mehr als 30% mit sehr hoher Wahrscheinlichkeit zur Entstehung einer Herzinsuffizienz führt.
Im der histologischen Aufarbeitung des Gehirns zeigen sich keine strukturellen Veränderungen bei herzkranken Mäusen, die die beobachteten Änderungen im Verhalten begründen könnten. Insbesondere kann eine hypoxische Hirnschädigung durch eine etwaige Minderperfusion empfindlicher Hirnareale ausgeschlossen werden.
Mäuse, die nach Induktion eines Myokardinfarktes eine Herzinsuffizienz entwickeln, zeigen nach 8 Wochen Depressions-assoziiertes, adynamisches Verhalten sowie eine Verminderung der kognitiven Leistungsfähigkeit, nicht aber Anzeichen von Angststörungen. Diesen Verhaltensänderungen kann kein strukturelles Korrelat im Gehirn zugewiesen werden. Dies ist ein Indiz dafür, dass sich Veränderung auf molekularer Ebene vollziehen, welche sich dem Mikroskop entziehen. Die im Myokard beobachtete Regulation des Serotoninstoffwechsels ist ein möglicher Erklärungsansatz hierfür.
Background:
While data from primary care suggest an insufficient control of vascular risk factors, little is known about vascular risk factor control in the general population. We therefore aimed to investigate the adoption of adequate risk factor control and its determinants in the general population free of cardiovascular disease (CVD).
Methods:
Data from the Characteristics and Course of Heart Failure Stages A-B and Determinants of Progression (STAAB) Cohort Study, a population-based study of inhabitants aged 30 to 79 years from the general population of Würzburg (Germany), were used. Proportions of participants without established CVD meeting targets for risk factor control recommended by 2016 ESC guideline were identified. Determinants of the accumulation of insufficiently controlled vascular risk factors (three or more) were assessed.
Results:
Between December 2013 and April 2015, 1379 participants without CVD were included; mean age was 53.1 ± 11.9 years and 52.9% were female; 30.8% were physically inactive, 55.2% overweight, 19.3% current smokers. Hypertension, dyslipidemia, and diabetes mellitus were prevalent in 31.8%, 57.6%, and 3.9%, respectively. Treatment goals were not reached despite medication in 52.7% of hypertensive, in 37.3% of hyperlipidemic and in 44.0% of diabetic subjects. Insufficiently controlled risk was associated with male sex (OR 1.94, 95%CI 1.44–2.61), higher age (OR for 30–39 years vs. 70–79 years 4.01, 95%CI 1.94–8.31) and lower level of education (OR for primary vs. tertiary 2.15, 95%CI 1.48–3.11).
Conclusions:
In the general population, prevalence of vascular risk factors was high. We found insufficient identification and control of vascular risk factors and a considerable potential to improve adherence to cardiovascular guidelines for primary prevention. Further studies are needed to identify and overcome patient- and physician-related barriers impeding successful control of vascular risk factors in the general population.
Background
Fabry disease (FD) is an X-linked recessive hereditary lysosomal storage disorder which results in the accumulation of globotriaosylceramid (Gb3) in tissues of kidney and heart as well as central and peripheral nervous system.
Besides prominent renal and cardiac organ involvement, cochlear symptoms like high-frequency hearing loss and tinnitus are frequently found with yet no comprehensive data available in the literature.
Objective
To examine hearing loss in patients with FD depending on cardiac and renal function.
Material and methods
Single-center study with 68 FD patients enrolled between 2012 and 2016 at the Department of Oto-Rhino-Laryngology, Plastic, Aesthetic and Reconstructive Head and Neck Surgery of the University of Würzburg. Every subject underwent an oto-rhino-laryngological examination as well as behavioral, electrophysiological and electroacoustical audiological testing. High-frequency thresholds were evaluated by using a modified PTA\(_{6}\) (0.5, 1, 2, 4, 6, 8) and HF-PTA (6, 8 kHz). Renal function was measured by eGFR, cardiac impairment was graduated by NYHA class.
Results
Sensorineural hearing loss was detected in 58.8% of the cohort, which occurred typically in sudden episodes and affected especially high frequencies. Hearing loss is asymmetric, beginning unilaterally and affecting the contralateral ear later. Tinnitus was reported by 41.2%. Renal and cardiac impairment influenced the severity of hearing loss (p < 0.05).
Conclusions
High frequency hearing loss is a common problem in patients with FD. Although not life-threatening, it can seriously reduce quality of life and should be taken into account in diagnosis and therapy. Optimized extensive hearing assessment including higher frequency thresholds should be used.
In their role as second messengers, cyclic nucleotides such as cAMP have a variety of intracellular effects. These complex tasks demand a highly organized orchestration of spatially and temporally confined cAMP action which should be best achieved by compartmentalization of the latter. A great body of evidence suggests that cAMP compartments may be established and maintained by cAMP degrading enzymes, e.g. phosphodiesterases (PDEs). However, the molecular and biophysical details of how PDEs can orchestrate cAMP gradients are entirely unclear. In this paper, using fusion proteins of cAMP FRET-sensors and PDEs in living cells, we provide direct experimental evidence that the cAMP concentration in the vicinity of an individual PDE molecule is below the detection limit of our FRET sensors (<100nM). This cAMP gradient persists in crude cytosol preparations. We developed mathematical models based on diffusion-reaction equations which describe the creation of nanocompartments around a single PDE molecule and more complex spatial PDE arrangements. The analytically solvable equations derived here explicitly determine how the capability of a single PDE, or PDE complexes, to create a nanocompartment depend on the cAMP degradation rate, the diffusive mobility of cAMP, and geometrical and topological parameters. We apply these generic models to our experimental data and determine the diffusive mobility and degradation rate of cAMP. The results obtained for these parameters differ by far from data in literature for free soluble cAMP interacting with PDE. Hence, restricted cAMP diffusion in the vincinity of PDE is necessary to create cAMP nanocompartments in cells.
Background/Aims:
Acute kidney injury (AKI) is a postoperative complication after cardiac surgery with a high impact on mortality and morbidity. Nephrocheck® [TIMP-2*IGFBP7] determines markers of tubular stress, which occurs prior to tubular damage. It is unknown at which time-point [TIMP-2*IGFBP7] measurement should be performed to ideally predict AKI. We investigated the association of [TIMP-2*IGFBP7] at various time-points with the incidence of AKI in patients undergoing elective cardiac surgery including cardio-pulmonary bypass.
Methods: In a prospective cohort study, serial blood and urine samples were collected from 150 patients: pre-operative, at ICU-admission, 24h and 48h post-surgery. AKI was defined as Serum-Creatinine rise >0.3 mg/dl within 48hrs. Urinary [TIMP-2*IGFBP7] was measured at pre-operative, ICU-admission and 24h post-surgery; medical staff was kept blinded to these results.
Results: A total of 35 patients (23.5%) experienced AKI, with a higher incidence in those with high [TIMP-2*IGFBP7] values at ICU admission (57.1% vs. 10.1%, p<0.001). In logistic regression [TIMP-2*IGFBP7] at ICU admission was independently associated with the occurrence of AKI (Odds Ratio 11.83; p<0.001, C-statistic= 0.74) after adjustment for EuroSCORE II and CBP-time.
Conclusions: Early detection of elevated [TIMP-2*IGFBP7] at ICU admission was strongly predictive for postoperative AKI and appeared to be more precise as compared to subsequent measurements.
Background
Aortic valve stenosis is a common finding diagnosed with high sensitivity in transthoracic echocardiography, but the examiner often finds himself confronted with uncertain results in patients with moderate pressure gradients and concomitant systolic heart failure. While patients with true-severe low-gradient aortic valve stenosis with either reduced or preserved left ventricular systolic function are primarily candidates for valve replacement, there is a relevant proportion of patients with pseudo-severe aortic valve stenosis anticipated not to benefit but actually rather deteriorate by interventional therapy or surgery.
Case presentation
In this article we present a case report of a male patient with pseudo-severe aortic valve stenosis due to cardiac amyloidosis highlighting the diagnostic schedule. The patient underwent stress echocardiography because of discrepant findings in transthoracic echocardiography and cardiac catheterization regarding the severity of aortic valve stenosis. After evaluation of the results, it became clear that he had a need for optimum heart failure medication and implantation of a cardiac resynchronization therapy defibrillator.
Conclusion
Due to the pitfalls in conventional as well as invasive diagnostics at rest, Stress echocardiography should be considered part of the standard optimum diagnostic spectrum in all unclear or borderline cases in order to confirm the correct diagnosis and constitute optimal therapy.
Background: Faster rates of cognitive decline are likely to result in earlier onset of cognitive impairment and dementia. d-galactose, a derivative of lactose, is used in animal studies to induce neurodegeneration. Milk is the primary source of lactose in the human diet, and its effects on cognitive decline have not been fully evaluated. Objective: Assess the association of milk intake with change in cognitive function over 20 years. Methods: A total of 13,751 participants of the Atherosclerosis Risk in Communities (ARIC) cohort completed a food frequency questionnaire and three neurocognitive evaluations from 1990 through 2013. Two single nucleotide polymorphisms (SNPs) were used to determine lactase persistence (LCT-13910 C/T for Whites and LCT-14010 G/C for Blacks). Mixed-effects models were used to study the association of milk intake with cognitive change. Multiple imputations by chained equations were used to account for attrition. Results: Milk intake greater than 1 glass/day was associated with greater decline in the global z-score over a 20-year period. The difference in decline was 0.10 (95% CI: 0.16, 0.03) z-scores, or an additional 10% decline, relative to the group reporting “almost never” consuming milk. Conclusions: Replication of these results is warranted in diverse populations with greater milk intake and higher variability of lactase persistence genotype.
Background
Arrhythmogenic cardiomyopathy is an inherited heart muscle disorder leading to ventricular arrhythmias and heart failure, mainly as a result of mutations in cardiac desmosomal genes. Desmosomes are cell-cell junctions mediating adhesion of cardiomyocytes; however, the molecular and cellular mechanisms underlying the disease remain widely unknown. Desmocollin-2 is a desmosomal cadherin serving as an anchor molecule required to reconstitute homeostatic intercellular adhesion with desmoglein-2. Cardiac specific lack of desmoglein-2 leads to severe cardiomyopathy, whereas overexpression does not. In contrast, the corresponding data for desmocollin-2 are incomplete, in particular from the view of protein overexpression. Therefore, we developed a mouse model overexpressing desmocollin-2 to determine its potential contribution to cardiomyopathy and intercellular adhesion pathology.
Methods and results
We generated transgenic mice overexpressing DSC2 in cardiac myocytes. Transgenic mice developed a severe cardiac dysfunction over 5 to 13 weeks as indicated by 2D-echocardiography measurements. Corresponding histology and immunohistochemistry demonstrated fibrosis, necrosis and calcification which were mainly localized in patches near the epi- and endocardium of both ventricles. Expressions of endogenous desmosomal proteins were markedly reduced in fibrotic areas but appear to be unchanged in non-fibrotic areas. Furthermore, gene expression data indicate an early up-regulation of inflammatory and fibrotic remodeling pathways between 2 to 3.5 weeks of age.
Conclusion
Cardiac specific overexpression of desmocollin-2 induces necrosis, acute inflammation and patchy cardiac fibrotic remodeling leading to fulminant biventricular cardiomyopathy.
High-Sensitivity Troponin: A Clinical Blood Biomarker for Staging Cardiomyopathy in Fabry Disease

Background
High‐sensitivity troponin (hs‐TNT), a biomarker of myocardial damage, might be useful for assessing fibrosis in Fabry cardiomyopathy. We performed a prospective analysis of hs‐TNT as a biomarker for myocardial changes in Fabry patients and a retrospective longitudinal follow‐up study to assess longitudinal hs‐TNT changes relative to fibrosis and cardiomyopathy progression.
Methods and Results
For the prospective analysis, hs‐TNT from 75 consecutive patients with genetically confirmed Fabry disease was analyzed relative to typical Fabry‐associated echocardiographic findings and total myocardial fibrosis as measured by late gadolinium enhancement (LE) on magnetic resonance imaging. Longitudinal data (3.9±2.0 years), including hs‐TNT, LE, and echocardiographic findings from 58 Fabry patients, were retrospectively collected. Hs‐TNT level positively correlated with LE (linear correlation coefficient, 0.72; odds ratio, 32.81 [95% CI, 3.56–302.59]; P=0.002); patients with elevated baseline hs‐TNT (>14 ng/L) showed significantly increased LE (median: baseline, 1.9 [1.1–3.3] %; follow‐up, 3.2 [2.3–4.9] %; P<0.001) and slightly elevated hs‐TNT (baseline, 44.7 [30.1–65.3] ng/L; follow‐up, 49.1 [27.6–69.5] ng/L; P=0.116) during follow‐up. Left ventricular wall thickness and EF of patients with elevated hs‐TNT were decreased during follow‐up, indicating potential cardiomyopathy progression.
Conclusions
hs‐TNT is an accurate, easily accessible clinical blood biomarker for detecting replacement fibrosis in patients with Fabry disease and a qualified predictor of cardiomyopathy progression. Thus, hs‐TNT could be helpful for staging and follow‐up of Fabry patients.
Multiple myeloma (MM) is a plasma cell malignancy with a significant heritable basis. Genome-wide association studies have transformed our understanding of MM predisposition, but individual studies have had limited power to discover risk loci. Here we perform a meta-analysis of these GWAS, add a new GWAS and perform replication analyses resulting in 9,866 cases and 239,188 controls. We confirm all nine known risk loci and discover eight new loci at 6p22.3 (rs34229995, P=1.31 × 10−8), 6q21 (rs9372120, P=9.09 × 10−15), 7q36.1 (rs7781265, P=9.71 × 10−9), 8q24.21 (rs1948915, P=4.20 × 10−11), 9p21.3 (rs2811710, P=1.72 × 10−13), 10p12.1 (rs2790457, P=1.77 × 10−8), 16q23.1 (rs7193541, P=5.00 × 10−12) and 20q13.13 (rs6066835, P=1.36 × 10−13), which localize in or near to JARID2, ATG5, SMARCD3, CCAT1, CDKN2A, WAC, RFWD3 and PREX1. These findings provide additional support for a polygenic model of MM and insight into the biological basis of tumour development.
Background
Diabetes mellitus (DM) is the leading cause of end-stage renal disease. Little is known about practice patterns of anti-diabetic therapy in the presence of chronic kidney disease (CKD) and correlates with glycaemic control. We therefore aimed to analyze current antidiabetic treatment and correlates of metabolic control in a large contemporary prospective cohort of patients with diabetes and CKD.
Methods
The German Chronic Kidney Disease (GCKD) study enrolled 5217 patients aged 18–74 years with an estimated glomerular filtration rate (eGFR) between 30–60 mL/min/1.73 m2 or proteinuria >0.5 g/d. The use of diet prescription, oral anti-diabetic medication, and insulin was assessed at baseline. HbA1c, measured centrally, was the main outcome measure.
Results
At baseline, DM was present in 1842 patients (35 %) and the median HbA1C was 7.0 % (25th–75th percentile: 6.8–7.9 %), equalling 53 mmol/mol (51, 63); 24.2 % of patients received dietary treatment only, 25.5 % oral antidiabetic drugs but not insulin, 8.4 % oral antidiabetic drugs with insulin, and 41.8 % insulin alone. Metformin was used by 18.8 %. Factors associated with an HbA1C level >7.0 % (53 mmol/mol) were higher BMI (OR = 1.04 per increase of 1 kg/m2, 95 % CI 1.02–1.06), hemoglobin (OR = 1.11 per increase of 1 g/dL, 95 % CI 1.04–1.18), treatment with insulin alone (OR = 5.63, 95 % CI 4.26–7.45) or in combination with oral antidiabetic agents (OR = 4.23, 95 % CI 2.77–6.46) but not monotherapy with metformin, DPP-4 inhibitors, or glinides.
Conclusions
Within the GCKD cohort of patients with CKD stage 3 or overt proteinuria, antidiabetic treatment patterns were highly variable with a remarkably high proportion of more than 50 % receiving insulin-based therapies. Metabolic control was overall satisfactory, but insulin use was associated with higher HbA1C levels.
Despite substantial advances in the imaging techniques and pathophysiological understanding over the last decades, identification of the underlying causes of left ventricular hypertrophy by means of echocardiographic examination remains a challenge in current clinical practice. The longitudinal strain bull’s eye plot derived from 2D speckle tracking imaging offers an intuitive visual overview of the global and regional left ventricular myocardial function in a single diagram. The bull’s eye mapping is clinically feasible and the plot patterns could provide clues to the etiology of cardiomyopathies. The present review summarizes the longitudinal strain, bull’s eye plot features in patients with various cardiomyopathies and concentric left ventricular hypertrophy and the bull’s eye plot features might serve as one of the cardiac workup steps on evaluating patients with left ventricular hypertrophy.
Objective:
Adrenal masses are incidentally discovered in 5% of CT scans. In 2013/2014, 81 million CT examinations were undertaken in the USA and 5 million in the UK. However, uncertainty remains around the optimal imaging approach for diagnosing malignancy. We aimed to review the evidence on the accuracy of imaging tests for differentiating malignant from benign adrenal masses. Design: A systematic review and meta-analysis was conducted.
Methods:
We searched MEDLINE, EMBASE, Cochrane CENTRAL Register of Controlled Trials, Science Citation Index, Conference Proceedings Citation Index, and ZETOC (January 1990 to August 2015). We included studies evaluating the accuracy of CT, MRI, or F-18-fluoro-deoxyglucose (FDG)-PET compared with an adequate histological or imaging-based follow-up reference standard.
Results:
We identified 37 studies suitable for inclusion, after screening 5469 references and 525 full-text articles. Studies evaluated the accuracy of CT (n = 16), MRI (n = 15), and FDG-PET (n = 9) and were generally small and at high or unclear risk of bias. Only 19 studies were eligible for meta-analysis. Limited data suggest that CT density >10 HU has high sensitivity for detection of adrenal malignancy in participants with no prior indication for adrenal imaging, that is, masses with <= 10 HU are unlikely to be malignant. All other estimates of test performance are based on too small numbers.
Conclusions:
Despite their widespread use in routine assessment, there is insufficient evidence for the diagnostic value of individual imaging tests in distinguishing benign from malignant adrenal masses. Future research is urgently needed and should include prospective test validation studies for imaging and novel diagnostic approaches alongside detailed health economics analysis.
The prevalence of diabetes around the world has reached epidemic proportions and is projected to increase to 642 million people by 2040. Diabetes is already the leading cause of end-stage kidney disease (ESKD) in most developed countries, and the growth in the number of people with ESKD around the world parallels the increase in diabetes. The presence of kidney disease is associated with a markedly elevated risk of cardiovascular disease and death in people with diabetes. Several new therapies and novel investigational agents targeting chronic kidney disease patients with diabetes are now under development. This conference was convened to assess our current state of knowledge regarding optimal glycemic control, current antidiabetic agents and their safety, and new therapies being developed to improve kidney function and cardiovascular outcomes for this vulnerable population.
Background
X-chromosomal inheritance patterns and generally rare occurrence of Fabry disease (FD) account for mono-mutational hemizygous male and heterozygous female patients. Female mutation carriers are usually clinically much less severely affected, which has been explained by a suggested mosaicism in cell phenotype due to random allele shutdown. However, clinical evidence is scarce and potential additional effects in female gene carriers, which might account for specific clinical characteristics such as less severe chronic kidney disease, are yet unknown.
Case presentation
This article reports on a 45 year old female patient carrying the two alpha-galactosidase A gene mutations c.416A > G, p.N139S in exon 3 and c.708G > C, p.W236C in exon 5, but still showing only mild organ manifestations.
Conclusion
This current case highlights the importance of careful clinical characterization in patients with Fabry disease, who may show additional rare constellations and, therefore, are in need of personalized medicine. The impact of potential additional protective effects exceeding the presence of a non-pathogenic GLA allele in female gene carriers requires further investigation.
Background
Fabry disease is characterized by a progressive deposition of sphingolipids in different organ systems, whereby cardiac involvement leads to death. We hypothesize that lysosomal storage of sphingolipids in the heart as occurring in Fabry disease does not reflect in higher cardiac lipid concentrations detectable by \(^1\)H magnetic resonance spectroscopy (MRS) at 3 Tesla.
Methods
Myocardial lipid content was quantified in vivo by \(^1\)H-MRS in 30 patients (12 male, 18 female; 18 patients treated with enzyme replacement therapy) with genetically proven Fabry disease and in 30 healthy controls. The study protocol combined \(^1\)H-MRS with cardiac cine imaging and LGE MRI in a single examination.
Results
Myocardial lipid content was not significantly elevated in Fabry disease (p = 0.225). Left ventricular (LV) mass was significantly higher in patients suffering from Fabry disease compared to controls (p = 0.019). Comparison of patients without signs of myocardial fibrosis in MRI (LGE negative; n = 12) to patients with signs of fibrosis (LGE positive; n = 18) revealed similar myocardial lipid content in both groups (p > 0.05), while the latter showed a trend towards elevated LV mass (p = 0.076).
Conclusions
This study demonstrates the potential of lipid metabolic investigation embedded in a comprehensive examination of cardiac morphology and function in Fabry disease. There was no evidence that lysosomal storage of sphingolipids influences cardiac lipid content as measured by \(^1\)H-MRS. Finally, the authors share the opinion that a comprehensive cardiac examination including three subsections (LGE; \(^1\)H-MRS; T\(_1\) mapping), could hold the highest potential for the final assessment of early and late myocardial changes in Fabry disease.
Lack of Ubiquitin Specific Protease 8 (USP8) Mutations in Canine Corticotroph Pituitary Adenomas

Purpose
Cushing’s disease (CD), also known as pituitary-dependent hyperadrenocorticism, is caused by adrenocorticotropic hormone (ACTH)-secreting pituitary tumours. Affected humans and dogs have similar clinical manifestations, however, the incidence of the canine disease is thousand-fold higher. This makes the dog an obvious model for studying the pathogenesis of pituitary-dependent hyperadrenocorticism. Despite certain similarities identified at the molecular level, the question still remains whether the two species have a shared oncogenetic background. Recently, hotspot recurrent mutations in the gene encoding for ubiquitin specific protease 8 (USP8) have been identified as the main driver behind the formation of ACTH-secreting pituitary adenomas in humans. In this study, we aimed to verify whether USP8 mutations also play a role in the development of such tumours in dogs.
Methods
Presence of USP8 mutations was analysed by Sanger and PCR-cloning sequencing in 38 canine ACTH-secreting adenomas. Furthermore, the role of USP8 and EGFR protein expression was assessed by immunohistochemistry in a subset of 25 adenomas.
Results
None of the analysed canine ACTH-secreting adenomas presented mutations in the USP8 gene. In a subset of these adenomas, however, we observed an increased nuclear expression of USP8, a phenotype characteristic for the USP8 mutated human tumours, that correlated with smaller tumour size but elevated ACTH production in those tumours.
Conclusions
Canine ACTH-secreting pituitary adenomas lack mutations in the USP8 gene suggesting a different genetic background of pituitary tumourigenesis in dogs. However, elevated nuclear USP8 protein expression in a subset of tumours was associated with a similar phenotype as in their human counterparts, indicating a possible end-point convergence of the different genetic backgrounds in the two species. In order to establish the dog as a useful animal model for the study of CD, further comprehensive studies are needed.
TRPC4α and TRPC4β Similarly Affect Neonatal Cardiomyocyte Survival during Chronic GPCR Stimulation

The Transient Receptor Potential Channel Subunit 4 (TRPC4) has been considered as a crucial Ca\(^{2+}\) component in cardiomyocytes promoting structural and functional remodeling in the course of pathological cardiac hypertrophy. TRPC4 assembles as homo or hetero-tetramer in the plasma membrane, allowing a non-selective Na\(^{+}\) and Ca\(^{2+}\) influx. Gαq protein-coupled receptor (GPCR) stimulation is known to increase TRPC4 channel activity and a TRPC4-mediated Ca\(^{2+}\) influx which has been regarded as ideal Ca\(^{2+}\) source for calcineurin and subsequent nuclear factor of activated T-cells (NFAT) activation. Functional properties of TRPC4 are also based on the expression of the TRPC4 splice variants TRPC4α and TRPC4β. Aim of the present study was to analyze cytosolic Ca\(^{2+}\) signals, signaling, hypertrophy and vitality of cardiomyocytes in dependence on the expression level of either TRPC4α or TRPC4β. The analysis of Ca\(^{2+}\) transients in neonatal rat cardiomyocytes (NRCs) showed that TRPC4α and TRPC4β affected Ca\(^{2+}\) cycling in beating cardiomyocytes with both splice variants inducing an elevation of the Ca\(^{2+}\) transient amplitude at baseline and TRPC4β increasing the Ca\(^{2+}\) peak during angiotensin II (Ang II) stimulation. NRCs infected with TRPC4β (Ad-C4β) also responded with a sustained Ca\(^{2+}\) influx when treated with Ang II under non-pacing conditions. Consistent with the Ca\(^{2+}\) data, NRCs infected with TRPC4α (Ad-C4α) showed an elevated calcineurin/NFAT activity and a baseline hypertrophic phenotype but did not further develop hypertrophy during chronic Ang II/phenylephrine stimulation. Down-regulation of endogenous TRPC4α reversed these effects, resulting in less hypertrophy of NRCs at baseline but a markedly increased hypertrophic enlargement after chronic agonist stimulation. Ad-C4β NRCs did not exhibit baseline calcineurin/NFAT activity or hypertrophy but responded with an increased calcineurin/NFAT activity after GPCR stimulation. However, this effect was not translated into an increased propensity towards hypertrophy but rather less hypertrophy during GPCR stimulation. Further analyses revealed that, although hypertrophy was preserved in Ad-C4α NRCs and even attenuated in Ad-C4β NRCs, cardiomyocytes had an increased apoptosis rate and thus were less viable after chronic GPCR stimulation. These findings suggest that TRPC4α and TRPC4β differentially affect Ca\(^{2+}\) signals, calcineurin/NFAT signaling and hypertrophy but similarly impair cardiomyocyte viability during GPCR stimulation.
Diagnosis of cardiac sarcoidosis is often challenging. Whereas cardiac magnetic resonance imaging (CMR) and positron emission tomography/computed tomography (PET/CT) with \(^{18}\)F-fluorodeoxyglucose (FDG) are most commonly used to evaluate patients, PET/CT using radiolabeled somatostatin receptor (SSTR) ligands for visualization of inflammation might represent a more specific alternative. This study aimed to investigate the feasibility of SSTR–PET/CT for detecting cardiac sarcoidosis in comparison to CMR.
15 patients (6 males, 9 females) with sarcoidosis and suspicion on cardiac involvement underwent SSTR-PET/CT imaging and CMR. Images were visually scored. The AHA 17-segment model of the left myocardium was used for localization and comparison of inflamed myocardium for both imaging modalities. In semi-quantitative analysis, mean (SUV\(_{mean}\)) and maximum standardized uptake values (SUV\(_{max}\)) of affected myocardium were calculated and compared with both remote myocardium and left ventricular (LV) cavity.
SSTR-PET was positive in 7/15, CMR in 10/15 patients. Of the 3 CMR+/PET- subjects, one patient with minor involvement (<25% of wall thickness in CMR) was missed by PET. The remaining two CMR+/PET- patients displayed no adverse cardiac events during follow-up.
In the 17-segment model, PET/CT yielded 27 and CMR 29 positive segments. Overall concordance of the 2 modalities was 96.1% (245/255 segments analyzed). SUV\(_{mean}\) and SUV\(_{max}\) in inflamed areas were 2.0±1.2 and 2.6±1.2, respectively. The lesion-to-remote myocardium and lesion-to-LV cavity ratios were 1.8±0.2 and 1.9±0.2 for SUV\(_{mean}\) and 2.0±0.3 and 1.7±0.3 for SUV\(_{max}\), respectively.
Detection of cardiac sarcoidosis by SSTR-PET/CT is feasible. Our data warrant further analysis in larger prospective series.
Zusammenfassend ist die Inzidenz von NNK bei Patienten mit einer chronischen
Nebennierenrindeninsuffizienz auch bei geschulten Patienten hoch. Das Auftreten der NNK
ist zudem mit einer erheblichen Morbidität und Mortalität verknüpft. In der heutigen Zeit
sterben weiterhin Patienten an den Folgen ihrer chronischen NNRI trotz einer adäquaten
Behandlung. Legt man die oben genannten Zahlen zugrunde, werden in den nächsten zehn
Jahren zwischen 5526 und 10647 Patienten an einer behandelbaren NNK versterben. Dies
macht die Notwendigkeit, die genauen Umstände und Ursachen einer NNK zu verstehen,
noch wichtiger. Die Analysen der Risikofaktoren einer NNK haben jedoch nur ein begrenztes
Potential, Patienten mit einem besonders hohen Risiko für das Auftreten von NKK zu
identifizieren. Patientenaufklärungen im Hinblick auf Dosisanpassungen der Glukokortikoide
in Stresssituationen werden die intravenösen Gaben von Glukokortikoiden nicht unnötig
machen, um eine drohende NNK zu verhindern. Anstrengungen um eine einheitliche
Patientenaufklärung mit dem Training der Selbstbehandlung mit parenteralem Hydrocortison
werden essentiell sein, um die NNK als Todesursache zu verhindern. Die aktuelle Studie
zeigt, dass die bisherigen Anstrengungen, eine einheitliche und breite Informationsgrundlage
zu vermitteln, nicht ausreichend sind. Denn auch bei medizinischem Fachpersonal besteht
anscheinend nach wie vor die Notwendigkeit einer intensiveren Schulung und Aufklärung. In
mehreren telefonischen Kontakten berichteten Patienten, dass sich ärztliche Kollegen nur
zurückhaltend auf eine ausreichend hohe intravenöse Cortisongabe einlassen konnten, in
einem Fall sogar, obwohl der betroffene Patient einen Notfallausweis bei sich trug. Auch in
der Laienpresse wurde von einem Fall berichtet, in dem laut Aussage der Patientin eine
Cortisonanpassung nicht ausreichend erfolgte.[40] Da die meisten tätigen Ärzte nur sehr
selten mit dem Krankheitsbild einer NNK konfrontiert sind, reagieren diese rezidivierend
nicht adäquat.[8] Auch zeigen sich die Symptome einer NNK häufig sehr unspezifisch,
weshalb es vielen Ärzten schwer fallen mag, diese zu erkennen. Da eine frühzeitige
Intervention für ein gutes Out Come jedoch unverzichtbar ist, sollten weitere Anstrengungen
gemacht werden, auch ärztliche Kollegen über diese Erkrankung und deren Behandlung
aufzuklären. Weiterhin zeigt sich auch die Schulung von Familie und Freunden als sinnvoll,
um das Aktionspotential im Falle einer akuten Verschlechterung der
Nebennierenrindenerkrankung zu verstärken.
423 Fragebögen waren für die Ersterhebung verfügbar, insgesamt schlossen 364 Patienten
(84%) das gesamte follow up über 2 Jahre ab. 767,5 Patientenjahre konnten über die Studie
erfasst werden. Innerhalb der Erhebungszeit wurde von 64 NNK berichtet. Dies entspricht
einer Häufigkeit von 8,3 Krisen/100 Patientenjahre. Als Hauptauslösefaktoren konnten
gastrointestinale Infektionen, Fieber und emotionaler Stress erkannt werden. Die Häufigkeit
entsprach 20%. Jedoch auch andere Stressoren konnten identifiziert werden. Hier zeigten sich
zum Beispiel Operationen, starke Schmerzen, Hitze, anstrengende körperliche Betätigung und
Schwangerschaft als ursächlich. In 7% der Fälle konnte bei einer plötzlichen
Verschlechterung des Zustandes keine auslösende Ursache gefunden werden. Es fand sich
jedoch, dass Patienten, die in ihrer Anamnese bereits eine oder mehrere NNK erlitten hatten,
ein höheres Risiko für eine erneute Entgleisung auswiesen. (Odds ratio 2,85, 95%
Konfidenzinterval 1.5–5.5, p 0,01) Andere Risikofaktoren konnten in der aktuellen Studie
nicht identifiziert werden. Während des Erhebungszeitraumes von zwei Jahren verstarben
insgesamt zehn Patienten. Vier dieser Todesfälle konnte einer NNK als Todesursache
zugeordnet werden. Dies entspricht einer Mortalitätshäufigkeit von 0,5/100 Patientenjahre.
Somit war in unserer Studie die NNK-assoziierte Mortalität 6% der NNK.
Unsere Studienteilnehmer wurden zu Beginn des Follow ups detailliert über die
Notwendigkeit von Dosisanpassungen und der Inanspruchnahme von professionellen Helfern
aufgeklärt. Dennoch zeigte sich im Vergleich zu anderen Studien mit 8,3 NNK/100
Patientenjahre keine reduzierte NNK-Häufigkeit. Jedoch konnte durchaus eine Reduktion der
NNK-Häufigkeit im Vergleich zu den Daten im Ersterhebungsbogen gezeigt werden. Neben
der Notwendigkeit, die Patientenaufklärung zu verbessern, zeigte die vorliegende Studie
jedoch auch, dass weitere Anstrengungen gemacht werden müssen, um das Vorgehen vor und
bei einer NNK weiterhin zu optimieren. So sind die genauen Umstände, die zu einer NNK
führen, bis heute noch nicht detailliert geklärt. Zwar konnten einige Risikofaktoren und
auslösende Situationen identifiziert werden, jedoch nicht die Frage, warum einige Patienten
eine NNK bekommen und Andere nicht. In diesem Zusammenhang besteht auch weiterhin die
Frage, warum einige Patienten gut auf eine Erhöhung der oralen Cortisondosis reagieren und
Andere trotz der Erhöhung in eine NNK kommen. Hieraus ergibt sich die Frage, ob es
Patienten gibt, die sensitiver auf das Cortison reagieren, und wenn dies der Fall ist, warum.
Hierfür könnte sprechen, dass es Patienten gibt, die rezidivierend in eine NNK kommen,
während Andere nie ein Krise erleiden. Auch wird es weiterhin Anstrengungen und neue
Strategien brauchen, eine schnellstmögliche Intervention im Falle einer Verschlechterung des
Allgemeinzustandes zu gewährleisten. Hier sind bessere, flächendeckende Aufklärungen für
medizinisches Fachpersonal und/oder die Verbesserung der mitgeführten Notfallkarten
notwendig. Eine Strategie, die Interventionszeit zu verkürzen, wäre der Ausbau der
Selbstinjektionen durch den betroffenen Patienten oder dessen Angehörige.
Ziel des Projektes war es, die klinische Beeinträchtigung der chronisch mild und moderat hyponatriämen Patienten in einem longitudinalen Vergleich vor und nach Elektrolytkorrektur zu erfassen, um neue Therapieansätze in der Behandlung der als asymptomatisch eingestuften Erkrankung zu erlangen.
Die Studie unterteilte sich in einen diagnostischen Teil, in welchem Spontanurin und eine Blutprobe analysiert, sowie die klinische Anamnese erfasst wurden, und einen experimentellen Teil, in welchem eine kognitive und neuromuskuläre Testung erfolgte. Die kognitive Untersuchung erfasste die Reaktionsschnelligkeit und Konzentrationsfähigkeit der Patienten. Hierfür standen die validierte Testbatterie zur Aufmerksamkeitsprüfung (TAP 2.0 für Windows) und das Softwarepaket Wiener Testsystem 32.0 zur Verfügung. Die neuromuskuläre Testung erfolgte mit Hilfe der Messplattform Balance-X-Sensor. Das Vorgehen war standardisiert und reproduzierbar.
Zusätzlich wurde anhand eines Anamnesebogens die Ausprägung klinischer Zeichen einer symptomatischen Hyponatriämie erfragt (Kopfschmerzen, Konfusion, mentale Verlangsamung, Reizbarkeit, Streitsüchtigkeit, Desorientierung zu Zeit/Ort/Person, Müdigkeit, Leistungsfähigkeit, Schwindel, Übelkeit, Erbrechen, Gangunsicherheit, Stürze, Krämpfe) und abschließend Daten über die aktuelle Lebensqualität der Patienten mit Hilfe des SF-36 Fragebogens erhoben.
Im Anschluss an die Testungen wurden die Patienten dazu angewiesen, ihren Natriumhaushalt mithilfe einer individuellen Medikation, die unter Beachtung der aktuellen Behandlungsrichtlinien ausgewählt wurde, zu korrigieren. Eine Wiedervorstellung der Patienten erfolgte nach 14 Tagen um die Testungen im elektrolytkorrigierten Zustand zu wiederholen.
Ein longitudinaler Vergleich der Messergebnisse soll Aufschluss über das Ausmaß einer Veränderung bezüglich der Leistungsfähigkeit und dem Empfinden der Patienten geben. Um den Querschnittsvergleich mit einem gesunden Kontrollkollektiv zu ermöglichen, fanden die neuromuskulären und kognitiven Testungen ebenfalls mit normonatriämen, gesunden Probanden statt. Für den Querschnittsvergleich der Lebensqualitätsmessung diente die international anerkannte US-Kontrollpopulation aus dem Jahr 1998.
Die Studie wurde zur übersichtlicheren Darstellung in zwei separate Arbeiten aufgeteilt. Der vorliegende Teil befasst sich mit der neuromuskulären Gleichgewichtstestung, sowie mit der Evaluierung der subjektiv empfundenen Lebensqualität der Patienten vor und nach Elektrolytkorrektur.
Im Rahmen dieser Studie konnten Daten über das Gleichgewicht und die subjektive Lebensqualität von insgesamt 19 Patienten mit chronischer Hyponatriämie (121-133 mmol/l) erhoben werden.
Die Auswertung des Gleichgewichtstests mittels Balance-X-Sensor wies auf ein reduziertes Gleichgewichtsvermögen der Patienten nach Elektrolytkorrektur hin. Hierbei konnte in keinem der ermittelten Parameter (Kraftvektorfläche, Muskelleistungsfrequenz, Stehleistung) ein signifikanter Unterschied gezeigt werden. Vorangegangene Studien zum verbesserten Gleichgewichtsvermögen nach medikamentöser Einstellung der Serumnatriumkonzentration konnten somit nicht bestätigt werden. Einzelne Stehübungen wurden zudem von dem gesunden Kontrollkollektiv mit einem signifikant besseren Gleichgewicht, bei zugleich niedrigerer Standardabweichung, ausgeführt.
Die Erfassung der Lebensqualität erfolgte mit Hilfe des SF-36 Fragebogens. Die Patienten wiesen nach Elektrolytkorrektur insgesamt keine signifikant verbesserte Lebensqualität auf. Im Querschnittsvergleich mit einer gesunden Kontrollpopulation wies die Auswertung der Fragebögen auf eine insgesamt verminderte Lebensqualität, sowohl vor als auch nach Elektrolytkorrektur, hin.
Die vorliegende Studie zeigt eine körperliche Beeinträchtigung, sowie eine tendenziell verschlechterte subjektive Lebensqualität, chronisch mild hyponatriämer Patienten. Eine signifikante Verbesserung der Werte konnte nach Anheben der Serumnatriumkonzentration nicht festgestellt werden. Die gewonnenen Ergebnisse sollten in Folgestudien mit einem größeren Patienten- und Probandenkollektiv, einem krankheitsspezifischen Fragebogen, sowie einem doppelblinden Studienaufbau überprüft werden. Auch die Möglichkeit der Durchführung einer multizentrischen Studie sollte geprüft werden. Der SF-36 Fragebogen und der Balance-X-Sensor können hierbei als kostengünstige Messinstrumente den subjektiven und objektiven Erfolg der Therapiemaßnahme dokumentieren. Ein konsequentes Anheben der Serumnatriumkonzentration bis in den Normbereich liefert zudem eine bessere Vergleichbarkeit mit internationalen Studien.
Das Gen CLEC16A ist mit der Autoimmunerkrankung Typ-1-Diabetes assoziiert. NOD-Mäuse mit einem Clec16a-KD sind vor der Entwicklung von Diabetes geschützt, der entscheidende Wirkungsort für Clec16a sind dabei TECs. Im Rahmen zentraler Toleranz präsentieren TECs CD4+ Thymozyten Selbstantigene auf MHC II-Komplexen. Autophagie ist ein Zellprozess, der in TECs MHC II-Komplexen Selbstantigene zuführt und so für die Entwicklung zentraler Toleranz essentiell ist. Das Ortholog von CLEC16A, ema, fördert die Bildung von Autophagosomen. So wurde vermutet, dass CLEC16A ein Suszeptibilitätsgen für Typ-1-Diabetes ist, weil es Autophagie in TECs und somit deren MHC II-Beladung verändert. Die vorliegende Arbeit schaltete CLEC16A in einer humanen Zelllinie durch RNAi aus und untersuchte die autophagische Aktivität dieser Zellen. Außerdem untersuchte sie die Autophagie von TECs aus NOD-Clec16a-KD-Mäusen. Die Beurteilung erfolgte morphologisch durch Immunzytochemie bzw. -histochemie und funktionell durch Immunoblots. Es wurde gezeigt, dass der KD von CLEC16A in vitro und in vivo Autophagie funktionell beeinträchtigt. Damit liefert die vorliegende Arbeit zusammen mit den Ergebnissen der Arbeitsgruppe Kissler einen möglichen Erklärungsansatz, warum CLEC16A ein mit Typ-1-Diabetes assoziiertes Gen ist. CLEC16A fördert Autophagie in TECs, was die Selbstantigen-Beladung von MHC II-Komplexen verändert. Selbstreaktive CD4+ Thymozyten führen so zum Verlust zentraler Toleranz und der Entwicklung von Typ-1-Diabetes. Weitere Untersuchungen sind jedoch notwendig, um diese Hypothese zu bekräftigen.
Für die Messung der Diffusionskapazität der Lunge für Kohlenmonoxid (Transferfak-tor) stehen verschiedene Verfahren zur Verfügung. Die Messwerte für den Transferfak-tor unterscheiden sich nicht nur je nach dem angewandten Verfahren, sondern auch in Abhängigkeit von der technischen Ausrüstung und den Eigenheiten der Methodik. Ziel dieser Arbeit war es, ein neu eingeführtes Rebreath-Gerät, das die Diffusionskapazität nach der von Stam (Stam et al. 1998) entwickelten Methode misst, in der klinischen Praxis zu testen. Die Messwerte sollten mit den Messungen nach dem Steady-State-Verfahren, das sich im Lungenfunktionslabor der Medizinischen Klinik und Poliklinik I der Universität Würzburg bewährt hatte, in Beziehung gesetzt werden. Durch adäquate Korrektur der Rebreath-Messwerte sollte eine möglichst gute Übereinstimmung der kor-respondierenden Messwerte erzielt werden.
Bei den untersuchten Patientenkollektiven handelte es sich um lungengesunde Proban-den und um Patienten mit obstruktiven bzw. restriktiven Lungenerkrankungen. Bei allen Kollektiven wurden Diffusionskapazitätsmessungen nach beiden Verfahren durchge-führt und parallel dazu auch spirometrische und bodyplethysmografische Untersuchun-gen vorgenommen. Die Auswertung der Messdaten umfasste zunächst eine univariate Analyse zur Ermittlung von diagnostischen und demografischen Einflussgrößen (Prä-diktoren) auf die Messwerte der beiden Verfahren und auf die Bodyplethysmographie. Anschließend wurden schrittweise mittels multipler linearer Regression aus den ver-schiedenen Einflussgrößen primäre Prädiktoren für die Messungen mit den beiden Ver-fahren ermittelt. Schließlich wurde eine Schätzformel abgeleitet, die unter der Berück-sichtigung der wichtigsten Prädiktoren die optimale Näherung der Rebreath-Messwerte an die korrespondierenden Steady-State-Messwerte erlaubte.
Mit beiden Verfahren wurden für die Patienten niedrigere Werte der Diffusionskapazi-tät ermittelt als für gesunde Probanden, mit den niedrigsten Werten bei Patienten mit restriktiver Lungenerkrankung. Für obstruktiv Erkrankte fanden sich die höchsten Alve-olarvolumina und entsprechend die niedrigsten Werte für den Krogh-Faktor. Mit beiden Verfahren konnte eine Abhängigkeit der Messwerte für den Transferfaktor von Ge-schlecht und Alter festgestellt werden. Als primärer Prädiktor galt allerdings in beiden Fällen die Diagnose. Bemerkenswert ist der starke Einfluss des BMI auf einige der ge-messenen Parameter (TLCOkorr, Krogh-Faktor, DLCO%), was eine stärkere Berück-sichtigung des BMI als prädiktiven Faktor nahe legt.
Die Korrelation zwischen den Messwerten aus den beiden Verfahren war mäßig. Das Steady-State-Gerät maß den Transferfaktor signifikant und wesentlich höher als das Rebreath-Gerät. Die schwächste Korrelation fand sich unter allen untersuchten Parame-tern für die Prozentwerte vom Soll TLCO% und DLCO%. Die Abweichung der korres-pondierenden Messwerte unterschied sich zudem je nach Diagnose, Alter und Höhe des Messwerts.
Die Spirometrie und Bodyplethysmografie zeigte die zu erwartenden geschlechts-, al-ters- und diagnosespezifischen Charakteristika, wobei nahezu alle bodyplethysmografi-schen Parameter primär mit der Diagnose und nur sekundär mit demografischen Fakto-ren korrelierten. Die Einsekundenkapazität FEV1 erwies sich als ein wichtiger Prädiktor für die Steady-State-Diffusionskapazität und als geeignet, um die Rebreath-Messwerte der Zielsetzung entsprechend zu korrigieren. Sowohl für die Absolut- als auch für Rela-tivwerte der Diffusionskapazität konnte eine Schätzformel abgeleitet werden, welche die optimale Näherung der Rebreath-Werte an die entsprechenden Steady-State-Werte ermöglichte. Die bessere Näherung gelang für die Absolutwerte des Transferfaktors.
Organ manifestations and long-term outcome of Fabry disease in patients with the GLA haplotype D313Y

Objectives: The severity of Fabry disease is dependent on the type of mutation in the α-galactosidase A (AgalA) encoding gene (GLA). This study focused on the impact of the GLA haplotype D313Y on long-term organ involvement and function.
Setting and participants: In this monocentric study, all participants presenting with the D313Y haplotype between 2001 and 2015 were comprehensively clinically investigated at baseline and during a 4-year follow-up if available. Five females and one male were included.
Primary and secondary outcome measures: Cardiac, nephrological, neurological, laboratory and quality of life data.
Results: AgalA enzyme activity in leucocytes (0.3±0.9 nmol/min/mg protein (mean±SD)) and serum lyso-Gb3 (0.6±0.3 ng/mL at baseline) were in normal range in all patients. Cardiac morphology and function were normal (left-ventricular (LV) ejection fraction 66±8%; interventricular septum 7.7±1.4 mm; LV posterior wall 7.5±1.4 mm; normalised LV mass in MRI 52±9 g/m2; LV global longitudinal strain −21.6±1.9%) and there were no signs of myocardial fibrosis in cardiac MRI. Cardiospecific biomarkers were also in normal range. Renal function was not impaired (estimated glomerular filtration rate MDRD 103±15 mL/min; serum-creatinine 0.75±0.07 mg/dL; cystatin-c 0.71±0.12 mg/L). One female patient (also carrying a Factor V Leiden mutation) had a transitory ischaemic attack. One patient showed white matter lesions in brain MRI, but none had Fabry-associated pain attacks, pain crises, evoked pain or permanent pain. Health-related quality of life analysis revealed a reduction in individual well-being. At long-term follow-up after 4 years, no significant change was seen in any parameter.
Conclusions: The results of the current study suggest that the D313Y genotype does not lead to severe organ manifestations as seen in genotypes known to be causal for classical FD."
Tyrosine kinase inhibitors represent today's treatment of choice in chronic myeloid leukemia (CML). Allogeneic hematopoietic stem cell transplantation (HSCT) is regarded as salvage therapy. This prospective randomized CML-study IIIA recruited 669 patients with newly diagnosed CML between July 1997 and January 2004 from 143 centers. Of these, 427 patients were considered eligible for HSCT and were randomized by availability of a matched family donor between primary HSCT (group A; N=166 patients) and best available drug treatment (group B; N=261). Primary end point was long-term survival. Survival probabilities were not different between groups A and B (10-year survival: 0.76 (95% confidence interval (CI): 0.69–0.82) vs 0.69 (95% CI: 0.61–0.76)), but influenced by disease and transplant risk. Patients with a low transplant risk showed superior survival compared with patients with high- (P<0.001) and non-high-risk disease (P=0.047) in group B; after entering blast crisis, survival was not different with or without HSCT. Significantly more patients in group A were in molecular remission (56% vs 39%; P = 0.005) and free of drug treatment (56% vs 6%; P<0.001). Differences in symptoms and Karnofsky score were not significant. In the era of tyrosine kinase inhibitors, HSCT remains a valid option when both disease and transplant risk are considered.
Proposals for enhanced health risk assessment and stratification in an integrated care scenario

Objectives
Population-based health risk assessment and stratification are considered highly relevant for large-scale implementation of integrated care by facilitating services design and case identification. The principal objective of the study was to analyse five health-risk assessment strategies and health indicators used in the five regions participating in the Advancing Care Coordination and Telehealth Deployment (ACT) programme (http://www.act-programme.eu). The second purpose was to elaborate on strategies toward enhanced health risk predictive modelling in the clinical scenario.
Settings
The five ACT regions: Scotland (UK), Basque Country (ES), Catalonia (ES), Lombardy (I) and Groningen (NL).

Responsible teams for regional data management in the five ACT regions.
Primary and secondary outcome measures
We characterised and compared risk assessment strategies among ACT regions by analysing operational health risk predictive modelling tools for population-based stratification, as well as available health indicators at regional level. The analysis of the risk assessment tool deployed in Catalonia in 2015 (GMAs, Adjusted Morbidity Groups) was used as a basis to propose how population-based analytics could contribute to clinical risk prediction.
Results
There was consensus on the need for a population health approach to generate health risk predictive modelling. However, this strategy was fully in place only in two ACT regions: Basque Country and Catalonia. We found marked differences among regions in health risk predictive modelling tools and health indicators, and identified key factors constraining their comparability. The research proposes means to overcome current limitations and the use of population-based health risk prediction for enhanced clinical risk assessment.
Conclusions
The results indicate the need for further efforts to improve both comparability and flexibility of current population-based health risk predictive modelling approaches. Applicability and impact of the proposals for enhanced clinical risk assessment require prospective evaluation.
Background
Chronic kidney disease (CKD) is a global health burden, yet it is still underrepresented within public health agendas in many countries. Studies focusing on the natural history of CKD are challenging to design and conduct, because of the long time-course of disease progression, a wide variation in etiologies, and a large amount of clinical variability among individuals with CKD. With the difference in health-related behaviors, healthcare delivery, genetics, and environmental exposures, this variability is greater across countries than within one locale and may not be captured effectively in a single study.
Methods
Studies were invited to join the network. Prerequisites for membership included: 1) observational designs with a priori hypotheses and defined study objectives, patient-level information, prospective data acquisition and collection of bio-samples, all focused on predialysis CKD patients; 2) target sample sizes of 1,000 patients for adult cohorts and 300 for pediatric cohorts; and 3) minimum follow-up of three years. Participating studies were surveyed regarding design, data, and biosample resources.
Results
Twelve prospective cohort studies and two registries covering 21 countries were included. Participants age ranges from >2 to >70 years at inclusion, CKD severity ranges from stage 2 to stage 5. Patient data and biosamples (not available in the registry studies) are measured yearly or biennially. Many studies included multiple ethnicities; cohort size ranges from 400 to more than 13,000 participants. Studies’ areas of emphasis all include but are not limited to renal outcomes, such as progression to ESRD and death.
Conclusions
iNET-CKD (International Network of CKD cohort studies) was established, to promote collaborative research, foster exchange of expertise, and create opportunities for research training. Participating studies have many commonalities that will facilitate comparative research; however, we also observed substantial differences. The diversity we observed across studies within this network will be able to be leveraged to identify genetic, behavioral, and health services factors associated with the course of CKD. With an emerging infrastructure to facilitate interactions among the investigators of iNET-CKD and a broadly defined research agenda, we are confident that there will be great opportunity for productive collaborative investigations involving cohorts of individuals with CKD.
Background
The X-chromosomally linked life-limiting Fabry disease (FD) is associated with deposits of the sphingolipid globotriaosylceramide 3 (Gb3) in various tissues. Skin is easily accessible and may be used as an additional diagnostic and follow-up medium. Our aims were to visualize skin Gb3 deposits in FD patients applying immunofluorescence and to determine if cutaneous Gb3 load correlates with disease severity.
Methods
At our Fabry Center for Interdisciplinary Therapy we enrolled 84 patients with FD and 27 healthy controls. All subjects underwent 5-mm skin punch biopsy at the lateral lower leg and the back. Skin samples were processed for immunohistochemistry using antibodies against CD77 (i.e. Gb3). Cutaneous Gb3 deposition was quantified in a blinded manner and correlated to clinical data.
Results
We found that Gb3 load was higher in distal skin of male FD patients compared to healthy controls (p<0.05). Men (p<0.01) and women (p<0.05) with a classic FD phenotype had higher distal skin Gb3 load than healthy controls. Men with advanced disease as reflected by impaired renal function, and men and women with small fiber neuropathy had more Gb3 deposits in distal skin samples than males with normal renal function (p<0.05) and without small fiber neuropathy. Gb3 deposits were not different between patients with and without enzyme replacement therapy.
Conclusions
Immunofluorescence on minimally invasive skin punch biopsies may be useful as a tool for assessment and follow-up in FD patients.
Mg\(^{2+}\) plays a vital role in platelet function, but despite implications for life-threatening conditions such as stroke or myocardial infarction, the mechanisms controlling [Mg\(^{2+}\)]i in megakaryocytes (MKs) and platelets are largely unknown. Transient receptor potential melastatin-like 7 channel (TRPM7) is a ubiquitous, constitutively active cation channel with a cytosolic α-kinase domain that is critical for embryonic development and cell survival. Here we report that impaired channel function of TRPM7 in MKs causes macrothrombocytopenia in mice (Trpm7\(^{fl/fl-Pf4Cre}\)) and likely in several members of a human pedigree that, in addition, suffer from atrial fibrillation. The defect in platelet biogenesis is mainly caused by cytoskeletal alterations resulting in impaired proplatelet formation by Trpm7\(^{fl/fl-Pf4Cre}\) MKs, which is rescued by Mg\(^{2+}\) supplementation or chemical inhibition of non-muscle myosin IIA heavy chain activity. Collectively, our findings reveal that TRPM7 dysfunction may cause macrothrombocytopenia in humans and mice.
The objective of this study was to identify unknown modulators of Calcineurin (Cn)-NFAT signaling. Measurement of NFAT reporter driven luciferase activity was therefore utilized to screen a human cardiac cDNA-library (~10\(^{7}\) primary clones) in C2C12 cells through serial dilutions until single clones could be identified. This extensive screening strategy culminated in the identification of SUMO2 as a most efficient Cn-NFAT activator. SUMO2-mediated activation of Cn-NFAT signaling in cardiomyocytes translated into a hypertrophic phenotype. Prohypertrophic effects were also observed in mice expressing SUMO2 in the heart using AAV9 (Adeno-associated virus), complementing the in vitro findings. In addition, increased SUMO2-mediated sumoylation in human cardiomyopathy patients and in mouse models of cardiomyopathy were observed. To decipher the underlying mechanism, we generated a sumoylation-deficient SUMO2 mutant (ΔGG). Surprisingly, ΔGG replicated Cn-NFAT-activation and the prohypertrophic effects of native SUMO2, both in vitro and in vivo, suggesting a sumoylation-independent mechanism. Finally, we discerned a direct interaction between SUMO2 and CnA, which promotes CnA nuclear localization. In conclusion, we identified SUMO2 as a novel activator of Cn-NFAT signaling in cardiomyocytes. In broader terms, these findings reveal an unexpected role for SUMO2 in cardiac hypertrophy and cardiomyopathy, which may open the possibility for therapeutic manipulation of this pathway.
Objective: This clinical practice guideline addresses the diagnosis and treatment of primary adrenal insufficiency. Participants: The Task Force included a chair, selected by The Clinical Guidelines Subcommittee of the Endocrine Society, eight additional clinicians experienced with the disease, a methodologist, and a medical writer. The co-sponsoring associations (European Society of Endocrinology and the American Association for Clinical Chemistry) had participating members. The Task Force received no corporate funding or remuneration in connection with this review. Evidence: This evidence-based guideline was developed using the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) system to determine the strength of recommendations and the quality of evidence.
Consensus Process: The evidence used to formulate recommendations was derived from two commissioned systematic reviews as well as other published systematic reviews and studies identified by the Task Force. The guideline was reviewed and approved sequentially by the Endocrine Society's Clinical Guidelines Subcommittee and Clinical Affairs Core Committee, members responding to a web posting, and the Endocrine Society Council. At each stage, the Task Force incorporated changes in response to written comments.
Conclusions: We recommend diagnostic tests for the exclusion of primary adrenal insufficiency in all patients with indicative clinical symptoms or signs. In particular, we suggest a low diagnostic (and therapeutic) threshold in acutely ill patients, as well as in patients with predisposing factors. This is also recommended for pregnant women with unexplained persistent nausea, fatigue, and hypotension. We recommend a short corticotropin test (250 mu g) as the "gold standard" diagnostic tool to establish the diagnosis. If a short corticotropin test is not possible in the first instance, we recommend an initial screening procedure comprising the measurement of morning plasma ACTH and cortisol levels. Diagnosis of the underlying cause should include a validated assay of autoantibodies against 21-hydroxylase. In autoantibody-negative individuals, other causes should be sought. We recommend once-daily fludrocortisone (median, 0.1 mg) and hydrocortisone (15-25 mg/d) or cortisone acetate replacement (20-35 mg/d) applied in two to three daily doses in adults. In children, hydrocortisone (similar to 8 mg/m\(^2\)/d) is recommended. Patients should be educated about stress dosing and equipped with a steroid card and glucocorticoid preparation for parenteral emergency administration. Follow-up should aim at monitoring appropriate dosing of corticosteroids and associated autoimmune diseases, particularly autoimmune thyroid disease.
Patients in the early stage of hypertensive heart disease tend to have normal echocardiographic findings. The aim of this study was to investigate whether pathology-specific echocardiographic morphologic and functional parameters can help to detect subclinical hypertensive heart disease. One hundred ten consecutive patients without a history and medication for arterial hypertension (AH) or other cardiac diseases were enrolled. Standard echocardiography and two-dimensional speckle tracking -imaging analysis were performed. Resting blood pressure (BP) measurement, cycle ergometer test (CET), and 24-hour ambulatory BP monitoring (ABPM) were conducted. Patients were referred to "septal bulge (SB)" group (basal-septal wall thickness >= 2 mm thicker than mid-septal wall thickness) or "no-SB" group. Echocardiographic SB was found in 48 (43.6%) of 110 patients. In this SB group, 38 (79.2%) patients showed AH either by CET or ABPM. In contrast, in the no-SB group (n = 62), 59 (95.2%) patients had no positive test for AH by CET or ABPM. When AH was solely defined by resting BP, SB was a reasonable predictive sign for AH (sensitivity 73%, specificity 76%). However, when AH was confirmed by CET or ABPM the echocardiographic SB strongly predicted clinical AH (sensitivity 93%, specificity 86%). In addition, regional myocardial deformation of the basal-septum in SB group was significantly lower than in no-SB group (14 +/- 4% vs. 17 +/- 4%; P < .001). In conclusion, SB is a morphologic echocardiographic sign for early hypertensive heart disease. Sophisticated BP evaluation including resting BP, ABPM, and CET should be performed in all patients with an accidental finding of a SB in echocardiography.
Estimation of absolute risk of cardiovascular disease (CVD), preferably with population-specific risk charts, has become a cornerstone of CVD primary prevention. Regular recalibration of risk charts may be necessary due to decreasing CVD rates and CVD risk factor levels. The SCORE risk charts for fatal CVD risk assessment were first calibrated for Germany with 1998 risk factor level data and 1999 mortality statistics. We present an update of these risk charts based on the SCORE methodology including estimates of relative risks from SCORE, risk factor levels from the German Health Interview and Examination Survey for Adults 2008–11 (DEGS1) and official mortality statistics from 2012. Competing risks methods were applied and estimates were independently validated. Updated risk charts were calculated based on cholesterol, smoking, systolic blood pressure risk factor levels, sex and 5-year age-groups. The absolute 10-year risk estimates of fatal CVD were lower according to the updated risk charts compared to the first calibration for Germany. In a nationwide sample of 3062 adults aged 40–65 years free of major CVD from DEGS1, the mean 10-year risk of fatal CVD estimated by the updated charts was lower by 29% and the estimated proportion of high risk people (10-year risk > = 5%) by 50% compared to the older risk charts. This recalibration shows a need for regular updates of risk charts according to changes in mortality and risk factor levels in order to sustain the identification of people with a high CVD risk.
Hyponatremia (HN) is a common condition, with a large number of etiologies and a complicated treatment. Although chronic HN has been shown to be a predictor of poor outcome, sodium-increasing treatments in chronic stable and asymptomatic HN have not proven to increase life expectancy. For symptomatic HN, in contrast, the necessity for urgent treatment has broadly been accepted to avoid the development of fatal cerebral edema. On the other hand, a too rapid increase of serum sodium in chronic HN may result in cerebral damage due to osmotic demyelinisation. Recently, administration of hypertonic saline bolus has been recommended as first-line treatment in patients with moderate-to-severe symptomatic HN. This approach is easy to memorize and holds the potential to greatly facilitate the initial treatment of symptomatic HN. First-line treatment of chronic HN is fluid restriction and if ineffective treatment with tolvaptan or in some patients other agents should be considered. A number of recommendations and guidelines have been published on HN. In the present review, the management of patients with HN in relation to everyday clinical practice is summarized with focus on the acute management.
Before the introduction of erythropoiesis-stimulating agents (ESAs) in 1989, repeated transfusions given to patients with end-stage renal disease caused iron overload, and the need for supplemental iron was rare. However, with the widespread introduction of ESAs, it was recognized that supplemental iron was necessary to optimize hemoglobin response and allow reduction of the ESA dose for economic reasons and recent concerns about ESA safety. Iron supplementation was also found to be more efficacious via intravenous compared to oral administration, and the use of intravenous iron has escalated in recent years. The safety of various iron compounds has been of theoretical concern due to their potential to induce iron overload, oxidative stress, hypersensitivity reactions, and a permissive environment for infectious processes. Therefore, an expert group was convened to assess the benefits and risks of parenteral iron, and to provide strategies for its optimal use while mitigating the risk for acute reactions and other adverse effects.
Patients with chronic kidney disease (CKD) exhibit an increased cancer risk compared to a healthy control population. To be able to estimate the cancer risk of the patients and to assess the impact of interventional therapies thereon, it is of particular interest to measure the patients’ burden of genomic damage. Chromosomal abnormalities, reduced DNA repair, and DNA lesions were found indeed in cells of patients with CKD. Biomarkers for DNA damage measurable in easily accessible cells like peripheral blood lymphocytes are chromosomal aberrations, structural DNA lesions, and oxidatively modified DNA bases. In this review the most common methods quantifying the three parameters mentioned above, the cytokinesis-block micronucleus assay, the comet assay, and the quantification of 8-oxo-7,8-dihydro-2′-deoxyguanosine, are evaluated concerning the feasibility of the analysis and regarding the marker’s potential to predict clinical outcomes.
Background
Agalsidase beta is a form of enzyme replacement therapy for Fabry disease, a genetic disorder characterised by low alpha-galactosidase A activity, accumulation of glycosphingolipids and life-threatening cardiovascular, renal and cerebrovascular events. In clinical trials, agalsidase beta cleared glycolipid deposits from endothelial cells within 6 months; clearance from other cell types required sustained treatment. We hypothesised that there might be a 'lag time' to clinical benefit after initiating agalsidase beta treatment, and analysed the incidence of severe clinical events over time in patients receiving agalsidase beta.
Methods
The incidence of severe clinical events (renal failure, cardiac events, stroke, death) was studied in 1044 adult patients (641 men, 403 women) enrolled in the Fabry Registry who received agalsidase beta (average dose 1 mg/kg every 2 weeks) for up to 5 years.
Results
The incidence of all severe clinical events was 111 per 1000 person-years (95% CI 84 to 145) during the first 6 months. After 6 months, the incidence decreased and remained stable within the range of 40-58 events per 1000 patient-years. The largest decrease in incidence rates was among male patients and those aged >= 40 years when agalsidase beta was initiated.
Conclusions
Contrary to the expected increased incidence of severe clinical events with time, adult patients with Fabry disease had decreased incidence of severe clinical events after 6 months treatment with agalsidase beta 1 mg/kg every 2 weeks.
Die Transplantation nimmt, mehr noch als die verschiedenen Dialyseverfahren, den höchsten Stellenwert in der Therapie eines terminalen Nierenversagens ein. Eine Transplantation erfordert jedoch auch immer eine suffiziente Immunsuppression, ohne die die Funktionalität des Transplantats nicht gewährleistet werden kann. Kortikosteroide werden dabei immer noch sehr häufig nach Nierentransplantation eingesetzt. Jedoch bringt diese Medikation auch Risiken mit sich. In der vorliegenden retrospektiven Analyse wurden die Daten von insgesamt 809 Patienten ausgewertet. Die Patienten wurden in zwei Gruppen aufgeteilt, wobei zwischen steroidfrei und nicht-steroidfrei in Jahr zwei nach Transplantation unterschieden wurde. Untersucht wurde, ob es Unterschiede im Patienten- und Transplantatüberleben, bei akuten Abstoßungen oder bei steroidtypischen Nebenwirkungen gibt.
Wir konnten ein tendenziell besseres Patientenüberleben in der steroidfreien Gruppe feststellen, vor allem zu sehen an der Zehn-Jahres-Überlebensrate (86,0% vs. 79,8%).
Bezüglich des Transplantatüberlebens zeigte sich ein statistisch signifikanter Unterschied zugunsten der steroidfreien Patienten (nach fünf Jahren 92,2% vs. 79,7%). Ein Transplantatverlust war in der steroidfreien Kohorte seltener zu beobachten (23,1% vs. 33,8%). Bei den in Jahr zwei steroidfreien Patienten ereigneten sich weniger akute Abstoßungsereignisse, sowohl bioptisch gesicherte als auch nur klinisch verdächtige Episoden.
Steroidtypische Nebenwirkungen, zum Beispiel ein Posttransplantationsdiabetes sowie kardiovaskuläre Ereignisse wurden tendenziell häufiger in der Kohorte mit fortgesetzter Steroideinnahme diagnostiziert. Im Auftreten von Malignomen gab es keinen Unterschied zwischen den beiden Gruppen. Zu beachten ist die Möglichkeit eines Selektionsbias bei der retrospektiven Auswertung.
Somit ergeben sich in unserer Analyse für eine verkürzte Einnahme von Steroiden nach Transplantation greifbare Vorteile für das Patienten- und Transplantatüberleben. Die Steroidtherapie sollte jedoch mindestens bis sechs Monate nach Transplantation beibehalten werden. Nach diesem Zeitraum erscheint das Absetzen der Steroide sicherer zu sein, da akute Abstoßungsepisoden seltener auftreten. Sowohl Patienten- und Transplantatüberleben werden durch das Absetzen der Steroidtherapie günstig beeinflusst. Ein früheres Absetzen, wie es in anderen Studien durchgeführt wurde, führte häufiger zu höheren Raten an akuten Rejektionen. Kortikosteroide waren und bleiben somit ein unverzichtbarer Teil der initialen immunsuppressiven Therapie nach Nierentransplantation.
Spiroergometrische Dauerbelastung von Probanden mit Morbus Addison, Diabetes mellitus Typ 1, Polyglandulärem Autoimmunsyndrom Typ 2 (erkrankt sowohl an Mb. Addison als auch an Diabetes mellitus Typ1) und gesunden Kontrollen. Blutzuckerverlauf, hormonelle Gegenregulation und kognitive Leistungsfähigkeit vor und nach Belastung wurden gemessen.
Während einer spiroergometrischen Dauerbelastung von 23 Minuten zeigte sich bei keinem der 10 Probanden mit ausschließlich Morbus Addison eine Neigung zur Hypoglykämie trotz fehlender Einnahme der mittäglichen Glukokortikoiddosis. Die Blutzucker blieben bei sämtlichen Probanden stabil und es zeigte sich sogar ein leichter Anstieg in der der Ergometrie anschließenden Nachbeobachtungsphase, eventuell als Hinweis auf eine mögliche Entwicklung einer Inulin-Resistenz.
Auf die erwartungsgemäße Mindersekretion von Adrenalin zeigte sich eine ame ehesten kompensatorisch leicht höhere Sekretion von Noradrenalin als bei den nebennierengesunden Gruppen. Die übrige Sekretion gegenregulatorischer Hormone entsprach den Vergleichsgruppen.
Die geleistete Arbeit am Fahrradergometer war bei den Probandengruppen mit Morbus Addison und APS 2 nahezu identisch, die Morbus Addison – Probanden traten sogar minimal weniger Ergometerwiderstand über die 15 Minuten Dauerbelastung. Dennoch zeigten die Probanden mit ausschließlich M. Addison einen adäquaten Anstieg der gegenregulatorischen Hormone ohne starke Schwankungen der Plasmaglukose, wohingegen es bei den Probanden mit APS 2, zu einem deutlichen Abfall der Plasmaglukose kam trotz deutlich niedrigerer Insulinkonzentrationen im Vergleich zur Probandengruppe mit ausschließlich Diabetes mellitus Typ 1. Die unzureichende Sekretion von Adrenalin, sowie der geringste Konzentrationsanstieg von Noradrenalin und dieser Untersuchung auch Wachstumshormon aller Probandengruppen verhinderte einen adäquaten Wiederanstieg des Blutzuckers.
Die Probanden mit Nebennierenrindeninsuffizienz verzeichneten teils signifikant schlechtere Ergebnisse bei einem Konzentrations- und einem Kurzzeitgedächtnistest im direkten Anschluss an die Ergometrie im Vergleich mit den anderen Probandengruppen.
Es gab keine relevanten Unterschiede der Testergebnisse in Ruhe. Die nebenniereninsuffizienten Probanden verbesserten sich jedoch signifikant weniger nach der Ergometrie bzw. zeigten nach dem Dauertest teils sogar schlechtere Leistungen. Die Probandengruppen mit Diabetes mellitus Typ 1 und die Kontrollgruppe zeigten eine erwartungsgemäße Verbesserung ihrer Leistung als Reaktion auf die vorherige körperliche Aktivität. Die Unterschiede in der kognitiven Performance sind am ehesten mit der unzureichenden Adrenalinsekretion und einem fehlenden akuten Cortisolanstieg der nebenniereninsuffizienten Probanden zu erklären.
Die Probanden mit Nebennierenrindeninsuffizienz wurden mit signifikant niedrigeren Widerständen am Fahrradergometer belastet als die nebennierengesunden Probanden. Ein möglicher Erklärungsansatz hierfür könnte eine gewisse cortisonbedingte Myopathie sein. Dies verdeutlicht nochmals die Notwendigkeit der Optimierung der Glukokortikoidsubstitutionstherapie. Neue Substitutionsregime sollten möglichst die physiologische circadiane Sekretionsrhythmik besser imitieren und im Optimalfall die Tagesdosis an Hydrocortison reduzieren, um glukokortikoidbedingte Nebenwirkungen wie Myopathie und Insulin-Resistenz zu reduzieren.
Die Probanden mit polyglandulärem Autoimmunsyndrom Typ 2, welche sowohl an Morbus Addison als auch an Diabetes mellitus Typ 1 leiden, müssen im Rahmen von Patientenschulungen besonders auf das Risiko von Hypoglykämien bei vermehrter körperlicher Aktivität hingewiesen werden. Patienten mit Insulinpumpe sollten das Ausschalten währenddessen erwägen und darüber hinaus besondere Aufmerksamkeit auf die Einnahme einer zusätzlichen Kohlenhydrateinheit für den Sport walten lassen. Eine zusätzliche Einnahme des Glukokortikoids ist in diesem Zusammenhang nicht sinnvoll. [31]
Ein vor dem Sport beispielsweise inhalativ appliziertes Epinephrinpräparat wäre eine mögliche Strategie zur Verbesserung des Plasmaglukose-Outcomes nach sportlicher Betätigung auf moderatem bzw. hohem Anstrengungslevel bei Patienten mit Morbus Addison und Diabetes mellitus Typ 1 und sollte Gegenstand weiterführender Studien sein.
High convection volume in online post-dilution haemodiafiltration: relevance, safety and costs

Increasing evidence suggests that treatment with online post-dilution haemodiafiltration (HDF) improves clinical outcome in patients with end-stage kidney disease, if compared with haemodialysis (HD). Although the primary analyses of three large randomized controlled trials (RCTs) showed inconclusive results, post hoc analyses of these and previous observational studies comparing online post-dilution HDF with HD showed that the risk of overall and cardiovascular mortality is lowest in patients who are treated with high-volume HDF. As such, the magnitude of the convection volume seems crucial and can be considered as the ‘dose’ of HDF. In this narrative review, the relevance of high convection volume in online post-dilution HDF is discussed. In addition, we briefly touch upon some safety and cost issues.
Background:
Recently, we gained evidence that impairment of rOat1 and rOat3 expression induced by ischemic acute kidney injury (AKI) is mediated by COX metabolites and this suppression might be critically involved in renal damage.
Methods:
(i) Basolateral organic anion uptake into proximal tubular cells after model ischemia and reperfusion (I/R) was investigated by fluorescein uptake. The putative promoter sequences from hOAT1 (SLC22A6) and hOAT3 (SCL22A8) were cloned into a reporter plasmid, transfected into HEK cells and (ii) transcriptional activity was determined after model ischemia and reperfusion as a SEAP reporter gen assay. Inhibitors or antagonists were applied with the beginning of reperfusion.
Results:
By using inhibitors of PKA (H89) and PLC (U73122), antagonists of E prostanoid receptor type 2 (AH6809) and type 4 (L161,982), we gained evidence that I/R induced down regulation of organic anion transport is mediated by COX1 metabolites via E prostanoid receptor type 4. The latter signaling was confirmed by application of butaprost (EP2 agonist) or TCS2510 (EP4 agonist) to control cells. In brief, the latter signaling was verified for the transcriptional activity in the reporter gen assay established. Therein, selective inhibitors for COX1 (SC58125) and COX2 (SC560) were also applied.
Conclusion:
Our data show (a) that COX1 metabolites are involved in the regulation of renal organic anion transport(ers) after I/R via the EP4 receptor and (b) that this is due to transcriptional regulation of the respective transporters. As the promoter sequences cloned were of human origin and expressed in a human renal epithelial cell line we (c) hypothesize that the regulatory mechanisms described after I/R is meaningful for humans as well.
Diabetic kidney disease (DKD) is the most common etiology of chronic kidney disease (CKD) in the industrialized world and accounts for much of the excess mortality in patients with diabetes mellitus. Approximately 45% of U.S. patients with incident end-stage kidney disease (ESKD) have DKD. Independent of glycemic control, DKD aggregates in families and has higher incidence rates in African, Mexican, and American Indian ancestral groups relative to European populations. The Family Investigation of Nephropathy and Diabetes (FIND) performed a genome-wide association study (GWAS) contrasting 6,197 unrelated individuals with advanced DKD with healthy and diabetic individuals lacking nephropathy of European American, African American, Mexican American, or American Indian ancestry. A large-scale replication and trans-ethnic meta-analysis included 7,539 additional European American, African American and American Indian DKD cases and non-nephropathy controls. Within ethnic group meta-analysis of discovery GWAS and replication set results identified genome-wide significant evidence for association between DKD and rs12523822 on chromosome 6q25.2 in American Indians (P = 5.74x10\(^{−9}\)). The strongest signal of association in the trans-ethnic meta-analysis was with a SNP in strong linkage disequilibrium with rs12523822 (rs955333; P = 1.31x10\(^{−8}\)), with directionally consistent results across ethnic groups. These 6q25.2 SNPs are located between the SCAF8 and CNKSR3 genes, a region with DKD relevant changes in gene expression and an eQTL with IPCEF1, a gene co-translated with CNKSR3. Several other SNPs demonstrated suggestive evidence of association with DKD, within and across populations. These data identify a novel DKD susceptibility locus with consistent directions of effect across diverse ancestral groups and provide insight into the genetic architecture of DKD.
Background:
Recent decades have seen a rise in the incidence of well-differentiated (mainly papillary) thyroid carcinoma around the world. In Germany, the age-adjusted incidence of well-differentiated thyroid carcinoma in 2010 was 3.5 per 100 000 men and 8.7 per 100 000 women per year.
Method:
This review is based on randomized, controlled trials and multicenter trials on the treatment of well-differentiated thyroid carcinoma that were retrieved by a selective literature search, as well as on three updated guidelines issued in the past two years.
Results:
The recommended extent of surgical resection depends on whether the tumor is classified as low-risk or high-risk, so that papillary microcar cinomas, which carry a highly favorable prognosis, will not be overtreated. More than 90% of localized, well-differentiated thyroid carcinomas can be cured with a combination of surgery and radioactive iodine therapy. Radio active iodine therapy is also effective in the treatment of well-differentiated thyroid carcinomas with distant metastases, yielding a 10-year survival rate of 90%, as long as there is good iodine uptake and the tumor goes into remission after treatment; otherwise, the 10-year survival rate is only 10%. In the past two years, better treatment options have become available for radioactive-iodine-resistant thyroid carcinoma. Phase 3 studies of two different tyrosine kinase inhibitors have shown that either one can markedly prolong progression-free survival, but not overall survival. Their more common clinically significant side effects are hand-foot syndrome, hypertension, diarrhea, proteinuria, and weight loss.
Conclusion:
Slow tumor growth, good resectability, and susceptibility to radioactive iodine therapy lend a favorable prognosis to most cases of well-differentiated thyroid carcinoma. The treatment should be risk-adjusted and interdisciplinary, in accordance with the current treatment guidelines. Even metastatic thyroid carcinoma has a favorable prognosis as long as there is good iodine uptake. The newly available medical treatment options for radioactive-iodine-resistant disease need to be further studied.
Natrium ist mit verschiedenen physiologischen Prozessen verbunden und spielt insbesondere in der Blutdruckregulation sowie in der Entstehung eines essentiellen Hypertonus eine bedeutende Rolle. Die täglich konsumierte Natriummenge der Gegenwart übersteigt den physiologisch notwendigen Natriumbedarf sowie den von den Fachgesellschaften empfohlenen Referenzwerten bedeutend. Exzessiver Natriumkonsum bei gleichzeitigem Vorliegen eines Bluthochdrucks erhöht das kardiovaskuläre Risiko beträchtlich. Eine Konsumbeschränkung in Verbindung mit adäquatem Blutdruckmanagment wird von zahlreichen Fachgesellschaften empfohlen. Zum Zeitpunkt der Untersuchung ist weitestgehend ungeklärt, ob Bluthochdruck im Alter sowie ein exzessiver Natriumkonsum mit oder ohne begleitendem Hypertonus die kognitive Funktionsfähigkeit beeinträchtigt.
Anhand der Daten der Women’s Health Initiative Memory Study, konnte vorliegende Arbeit einen eindeutigen Zusammenhang zwischen dem Vorliegen eines Bluthochdrucks und dem Auftreten milder kognitiver Beeinträchtigung bei postmenopausalen Frauen ≥ 60 Jahre zeigen. Gleichzeitig weist vorliegende Untersuchung auf einen möglichen Zusammenhang zwischen erhöhtem Natriumkonsum und spätere kognitive Funktionsfähigkeit bei hypertensiven Frauen hin. Während eine natriumarme Ernährung das Risiko für kognitiven Abbau bei hypertensiven Individuen nicht erhöhte, wurde eine Assoziation zwischen natriumreicher Ernährung und einem gesteigerten Risiko für kognitive Verschlechterung beobachtet. Allerdings waren diese Ergebnisse nicht signifikant und bedürfen weiterer Überprüfung. Auswirkungen einer natriumreichen Ernährung auf die kognitive Funktionsfähigkeit bei normotensiven Frauen wurden nicht gefunden.
Die Resultate dieser Untersuchung unterstreichen den über die kardiovaskuläre Prävention hinaus reichenden Stellenwert einer adäquaten Blutdruckregulation im Allgemeinen sowie einer Natriumrestriktion bei hypertensiven Individuen im Besonderen zum Erhalt kognitiver Funktionsfähigkeit.
FAZIT:
Die EQUAL-Studie stellt eine europäische Initiative zur Beantwortung wichtiger Fragen rund um die Betreuung älterer Patienten mit fortschreitender chronischen Niereninsuffizienz dar.
Die Pilotstudie konnte in Deutschland erfolgreich durchgeführt werden. Es konnten insgesamt 30 Patienten eingeschlossen werden. Hierbei wurden geeignete Rekrutierungsarten und Rekrutierungsstrategien identifiziert.
Die Hauptstudie konnte mit Modifikationen im Design und Organisation aktuell erfolgreich in Deutschland und Europa durchgeführt werden.
Immune cells (IC) play a crucial role in murine stroke pathophysiology. However, data are limited on the role of these cells in ischemic stroke in humans. We therefore aimed to characterize and compare peripheral IC subsets in patients with acute ischemic stroke/transient ischemic attack (AIS/TIA), chronic cerebrovascular disease (CCD) and healthy volunteers (HV). We conducted a case-control study of patients with AIS/TIA (n = 116) or CCD (n = 117), and HV (n = 104) who were enrolled at the University Hospital Würzburg from 2010 to 2013. We determined the expression and quantity of IC subsets in the three study groups and performed correlation analyses with demographic and clinical parameters. The quantity of several IC subsets differed between the AIS/TIA, CCD, and HV groups. Several clinical and demographic variables independently predicted the quantity of IC subsets in patients with AIS/TIA. No significant changes in the quantity of IC subsets occurred within the first three days after AIS/TIA. Overall, these findings strengthen the evidence for a pathophysiologic role of IC in human ischemic stroke and the potential use of IC-based biomarkers for the prediction of stroke risk. A comprehensive description of IC kinetics is crucial to enable the design of targeted treatment strategies.
EINLEITUNG: Die frühzeitige Erregeridentifikation bei Sepsis-Patienten ist essentiell zur Therapieoptimierung und Senkung der Letalität. Molekularbiologische Detektionsmethoden mit direktem Nachweis bakterieller oder fungaler DNA aus Vollblut stellen einen vielversprechenden Ansatz dar, mit kürzerer Zeitdauer bis zum Resultat und potentiell erhöhter Sensitivität. Beim Vergleich dieser PCR-basierten, kulturunabhängigen Verfahren mit der konventionellen Blutkultur muss streng zwischen antibiotisch vorbehandelten und antibiotisch nicht vorbehandelten Patienten unterschieden werden.
METHODIK: Bei Patienten, die sich von Mai 2010 bis Dezember 2011 mit V.a. Sepsis im Zentrum für Innere Medizin einer Universitätsklinik vorstellten, wurden im Rahmen der IMPACT Sepsis Studie zusätzlich zum routinemäßigen Vorgehen 2 x 5 ml EDTA Blut für die VYOO®-PCR entnommen. In der vorliegenden Arbeit wurden die Erregernachweise der PCR mit den Ergebnissen der Blutkultur für alle antibiotisch nicht vorbehandelten Patienten hinsichtlich Detektionsrate, Time to Result und Plausibilität verglichen. Außerdem wurde die antibiotische Therapie dieser Patienten analysiert und potentielle Therapieoptimierungen durch die PCR-Ergebnisse evaluiert.
ERGEBNISSE: 126 der 200 in die IMPACT Sepsis Studie eingeschlossenen Patienten waren nicht antibiotisch vorbehandelt. Ihr Durchschnittsalter betrug 66,0 ± 16,4 (MW ± SD) Jahre, der Anteil männlicher Patienten 60% und der Anteil immunsupprimierter Patienten 33%. Die durchschnittliche Krankenhaus-Liegedauer lag bei 11,9 ± 10,5 (MW ± SD) Tagen, der Anteil der Patienten mit schwerer Sepsis oder septischem Schock bei 47% und die Letalitätsrate bei 9,7%. Die durchschnittliche Latenzzeit bis zur ersten Antibiotika-Gabe betrug 4,13 ± 6,75 (MW ± SD) h bei einem Median von 2,16 h.
Insgesamt wurden 26 Erreger identifiziert. In 6 Fällen wurde der Erreger von beiden Methoden identifiziert, in 15 nur von der Blutkultur und in 5 nur von der PCR. Die Detektionsraten betrugen 8,7% für die PCR und 16,7% für die Blutkultur (Fisher-Yates-Test; p=0,087; korrigiertes p=1). Die Zeitdauer bis zum Erregerresultat war bei der PCR signifikant kürzer (8,0h bzw. 40,0h; korrigiertes p<0,001). Die PCR versagte vor allem beim Nachweis von Streptokokken, während die Blutkultur mehrere, teilweise gramnegative Problemkeime nicht erfasste. Bei mindestens 4% aller Patienten, 9% der Patienten mit schweren Verlaufsformen und 45% der Patienten mit positivem PCR-Resultat hätte eine Berücksichtigung des PCR-Ergebnisses höchstwahrscheinlich zu einer Therapieoptimierung beigetragen.
SCHLUSSFOLGERUNG: Die beiden untersuchten Verfahren zur Erregerdiagnostik unterschieden sich hinsichtlich der Detektionsrate nicht signifikant, eine diagnostische Überlegenheit der VYOO®-PCR gegenüber der Blutkultur konnte also nicht festgestellt werden. Als komplementäres Verfahren zusätzlich zur Blutkultur bei ausgewählten Patientengruppen eingesetzt, kann durch die PCR eine Verbesserung des therapeutischen Managements von Sepsis-Patienten erzielt werden.
Background
Solitary metastases to the pancreas are rare. Therefore the value of resection in curative intention remains unclear. In the literature there are several promising reports about resection of solitary metastasis to the pancreas mainly of renal origin.
Case presentation
Here we report for the first time on the surgical therapy of a 1.5 cm solitary pancreatic metastasis of an adrenocortical carcinoma. The metastasis occurred almost 6 years after resection of the primary tumor. A partial pancreatoduodenectomy was performed and postoperatively adjuvant mitotane treatment was initiated. During the follow-up of 3 years after surgery no evidence of tumor recurrence occurred.
Conclusion
Resection of pancreatic tumors should be considered, even if the mass is suspicious for metastatic disease including recurrence of adrenocortical cancer.
Balanced hydroxyethylstarch (HES 130/0.4) impairs kidney function in-vivo without inflammation

Volume therapy is a standard procedure in daily perioperative care, and there is an ongoing discussion about the benefits of colloid resuscitation with hydroxyethylstarch (HES). In sepsis HES should be avoided due to a higher risk for acute kidney injury (AKI). Results of the usage of HES in patients without sepsis are controversial. Therefore we conducted an animal study to evaluate the impact of 6% HES 130/0.4 on kidney integrity with sepsis or under healthy conditions Sepsis was induced by standardized Colon Ascendens Stent Peritonitis (sCASP). sCASP-group as well as control group (C) remained untreated for 24 h. After 18 h sCASP+HES group (sCASP+VOL) and control+HES (C+VOL) received 50 ml/KG balanced 6% HES (VOL) 130/0.4 over 6h. After 24h kidney function was measured via Inulin- and PAH-Clearance in re-anesthetized rats, and serum urea, creatinine (crea), cystatin C and Neutrophil gelatinase-associated lipocalin (NGAL) as well as histopathology were analysed. In vitro human proximal tubule cells (PTC) were cultured +/- lipopolysaccharid (LPS) and with 0.1–4.0% VOL. Cell viability was measured with XTT-, cell toxicity with LDH-test. sCASP induced severe septic AKI demonstrated divergent results regarding renal function by clearance or creatinine measure focusing on VOL. Soleley HES (C+VOL) deteriorated renal function without sCASP. Histopathology revealed significantly derangements in all HES groups compared to control. In vitro LPS did not worsen the HES induced reduction of cell viability in PTC cells. For the first time, we demonstrated, that application of 50 ml/KG 6% HES 130/0.4 over 6 hours induced AKI without inflammation in vivo. Severity of sCASP induced septic AKI might be no longer susceptible to the way of volume expansion
In dieser post-hoc Analyse der 4D Studie wurde evaluiert, in welchem Maße endotheliale Dysfunktion und Inflammation für die deutlich erhöhte kardiovaskuläre Morbidität und Mortalität terminal niereninsuffizienter Patienten verantwortlich sind. Das untersuchte Patientenkollektiv bestand aus 1255 Hämodialyse-Patienten mit Diabetes mellitus Typ 2, die multizentrisch, randomisiert und kontrolliert mit 20 mg Atorvastatin pro Tag oder Placebo behandelt wurden. Nutzen und Risiken dieser Therapie in Hinblick auf den primären Endpunkt bestehend aus kardialem Tod, Myokardinfarkt und Schlaganfall wurden über einen medianen follow-up Zeitraum von vier Jahren evaluiert. Bei diesem Kollektiv wurden die Biomarker ADMA, SDMA, H-Arginin und SCRP gemessen und mit Hilfe eines multivariaten Cox-Regressionsmodells hinsichtlich ihres prädiktiven Wertes in Bezug auf Schlaganfall und kardiovaskuläre Ereignisse untersucht. SCRP wurde zusätzlich in Relation zu dem traditionellen kardiovaskulären Biomarker LDL-Cholesterin analysiert und der Einfluss von Atorvastatin auf die untersuchten Marker erforscht.
Im Folgenden werden die wichtigsten Ergebnisse zusammengefasst.
Patienten der zweiten Quartile (ADMA >0,77 bis ≤0,86 µmol L-1) hatten ein um 66% höheres Risiko einen Myokardinfarkt (HR 1.66, 95% KI 1.12-2.46) und ein um 36% höheres Risiko den kombinierten kardiovaskulären Endpunkt zu erreichen (HR 1.36, 95% KI 1.05-1.77) als Patienten der ersten Quartile (ADMA ≤0,77 µmol L-1).
SDMA war mit einer erhöhten Inzidenz von Schlaganfällen verbunden. Das Risiko einen Schlaganfall zu erleiden war für Patienten aus Quartile 3 (SDMA >2,45 bis ≤2,96 µmol L-1) um 125% (HR 2.25, 95% KI 1.24-4.11) und für Patienten aus Quartile 4 (SDMA >2,96 µmol L-1) um 90% (HR 1.89, 95% KI 1.02-3.53) höher als für Patienten aus Quartile 1.
In Abhängigkeit des H-Arginin-Wertes sank das Risiko des plötzlichen Herztodes und der Gesamtmortalität: Patienten in Quartile 4 (>1,4 µmol L-1) hatten ein um 51% niedrigeres Risiko an plötzlichem Herztod zu versterben als Patienten in Quartile 1 (≤0,87 µmol L-1) (HR 0.49, 95% KI 0.29-0.85). Das Risiko der Gesamtmortalität war in Quartile 3 (>1,1 bis ≤1,4 µmol L-1) (HR 0.78, 95% KI 0.62-0.99) und 4 (HR 0.65, 95% KI 0.50-0.84) im Vergleich zu Quartile 1 am niedrigsten.
Sensitives CRP als Baseline-Parameter wurde als Prädiktor für den kombinierten kardiovaskulären Endpunkt (HR 1.10, 95% KI 1.01-1.18) und die Gesamtmortalität (HR 1.25, 95% KI 1.17-1.33) identifiziert. Ebenso war der Mittelwert zweier Messungen mit einem erhöhten kardiovaskulären Risiko assoziiert: Das Risiko einen Schlaganfall (HR 1.20, 95% KI 1.01-1.42) oder den plötzlichen Herztod (HR 1.20, 95% KI 1.05-1.37) zu erleiden stieg um 20% pro Einheit Anstieg in logarithmisch-transformiertem CRP, das den kombinierten kardiovaskulären Endpunkt zu erreichen um 10% (HR 1.10, 95% KI 1.02-1.20) und die Gesamtmortalität um 30% (HR 1.30, 95% KI 1.21-1.39). Patienten, deren SCRP-Wert initial unterhalb des Medians von 5 mg L-1 lag und im Verlauf um mehr als 100% anstieg, hatten ein um mehr als 4fach erhöhtes Risiko einen Schlaganfall zu erleiden (HR 4.07, 95% KI 1.20-13.78) und ein um mehr als 60% erhöhtes Risiko zu versterben (HR 1.63, 95% KI 1.08-2.45) als Patienten, deren SCRP im Verlauf um 47,3% fiel.
LDL in Kombination mit SCRP zeigte sich hier nicht prädiktiv für kardiovaskuläre Ereignisse.
Die Atorvastatin-Behandlung beeinflusste außer dem LDL-Cholesterin keinen der untersuchten Parameter. In der Subgruppe der Patienten mit den höchsten CRP-Werten fand sich eine erhöhte Schlaganfallinzidenz unter denen, die mit Atorvastatin behandelt wurden.
Insgesamt stellen hohes SDMA und SCRP als Verlaufsparameter im Gegensatz zu ADMA und H-Arginin unabhängige Prädiktoren für das Auftreten von Schlaganfällen bei Hämodialyse-Patienten mit Diabetes mellitus Typ 2 dar.
We are reporting a long-time magnetic resonance imaging (MRI) follow-up in a rare case of cardiac left lateral wall hypertrophy. Hypertrophic cardiomyopathy (HCM) is the most common genetic cardiovascular disorder and a significant cause of sudden cardiac death. Cardiac magnetic resonance (CMR) imaging can be a valuable tool for assessment of detailed information on size, localization, and tissue characteristics of hypertrophied myocardium. However, there is still little knowledge of long-term evolution of HCM as visualized by magnetic resonance imaging. Recently, our group reported a case of left lateral wall HCM as a rare variant of the more common forms, such as septal HCM, or apical HCM. As we now retrieved an old cardiac MRI acquired in this patient more than 20 years ago, we are able to provide the thrilling experience of an ultra-long MRI follow-up presentation in this rare case of left lateral wall hypertrophy. Furthermore, this case outlines the tremendous improvements in imaging quality within the last two decades of CMR imaging.
Background
“Tako-Tsubo cardiomyopathy” (TTC) is a syndrome characterized by left ventricular (LV) wall motion abnormalities, usually without coronary artery disease, mimicking the diagnosis of acute coronary syndrome. It most often affects post-menopausal women and TTC tends to run a benign course with very low rates of recurrence, complications or mortality. The condition is also called “stress-induced cardiomyopathy” because acute physical or emotional stress appears to be frequently related to its onset. The pathogenic role of premorbid or comorbid psychiatric illnesses has been discussed controversially. For the first time, we present a case of fourfold recurrent TTC with severe complications in a pre-menopausal woman. Furthermore, a long history of flaring posttraumatic stress symptoms anteceded the first event.
Case presentation
A 43-year old, pre-menopausal Caucasian woman was hospitalized with symptoms of acute coronary syndrome. Clinical examination revealed hypokinetic wall motion in the apical ventricular region with no signs of coronary artery disease and diagnosis of TTC was established. She experienced recurrence three times within the following ten months, which led to thrombembolism and myocardial scarring among others. The circumstances of chronic distress were striking. 16 years ago she miscarried after having removed a myoma according to her doctor’s suggestion. Since then, she has suffered from symptoms of posttraumatic distress which peaked annually at the day of abortion. Chronic distress became even more pronounced after the premature birth of a daughter some years later. The first event of TTC occurred after a family dispute about parenting.
Conclusion
This is the first case report of fourfold TTC in a pre-menopausal woman. From somatic perspectives, the course of the disease with recurrences and complications underlines the fact that TTC is not entirely benign. Furthermore, it is the first case report of long lasting symptoms of traumatic stress anteceding TTC. Close connections between adrenergic signaling and late onset of clinical stress symptoms are well known in the psychopathology of traumatization. Although larger clinical trials are needed to elucidate possible interactions of premorbid psychiatric illnesses and TTC, cardiologists should be vigilant especially in cases of recurrent TTC.
Background
Information extraction techniques that get structured representations out of unstructured data make a large amount of clinically relevant information about patients accessible for semantic applications. These methods typically rely on standardized terminologies that guide this process. Many languages and clinical domains, however, lack appropriate resources and tools, as well as evaluations of their applications, especially if detailed conceptualizations of the domain are required. For instance, German transthoracic echocardiography reports have not been targeted sufficiently before, despite of their importance for clinical trials. This work therefore aimed at development and evaluation of an information extraction component with a fine-grained terminology that enables to recognize almost all relevant information stated in German transthoracic echocardiography reports at the University Hospital of Würzburg.
Methods
A domain expert validated and iteratively refined an automatically inferred base terminology. The terminology was used by an ontology-driven information extraction system that outputs attribute value pairs. The final component has been mapped to the central elements of a standardized terminology, and it has been evaluated according to documents with different layouts.
Results
The final system achieved state-of-the-art precision (micro average.996) and recall (micro average.961) on 100 test documents that represent more than 90 % of all reports. In particular, principal aspects as defined in a standardized external terminology were recognized with f 1=.989 (micro average) and f 1=.963 (macro average). As a result of keyword matching and restraint concept extraction, the system obtained high precision also on unstructured or exceptionally short documents, and documents with uncommon layout.
Conclusions
The developed terminology and the proposed information extraction system allow to extract fine-grained information from German semi-structured transthoracic echocardiography reports with very high precision and high recall on the majority of documents at the University Hospital of Würzburg. Extracted results populate a clinical data warehouse which supports clinical research.
Background
Anemia is common and is associated with impaired clinical outcomes in diabetic chronic kidney disease (CKD). It may be explained by reduced erythropoietin (EPO) synthesis, but recent data suggest that EPO-resistance and diminished iron availability due to inflammation contribute significantly. In this cohort study, we evaluated the impact of hepcidin-25—the key hormone of iron-metabolism—on clinical outcomes in diabetic patients with CKD along with endogenous EPO levels.
Methods
249 diabetic patients with CKD of any stage, excluding end-stage renal disease (ESRD), were enrolled (2003–2005), if they were not on EPO-stimulating agent and iron therapy. Hepcidin-25 levels were measured by radioimmunoassay. The association of hepcidin-25 at baseline with clinical variables was investigated using linear regression models. All-cause mortality and a composite endpoint of CKD progression (ESRD or doubling of serum creatinine) were analyzed by Cox proportional hazards models.
Results
Patients (age 67 yrs, 53% male, GFR 51 ml/min, hemoglobin 131 g/L, EPO 13.5 U/L, hepcidin-25 62.0 ng/ml) were followed for a median time of 4.2 yrs. Forty-nine patients died (19.7%) and forty (16.1%) patients reached the composite endpoint. Elevated hepcidin levels were independently associated with higher ferritin-levels, lower EPO-levels and impaired kidney function (all p<0.05). Hepcidin was related to mortality, along with its interaction with EPO, older age, greater proteinuria and elevated CRP (all p<0.05). Hepcidin was also predictive for progression of CKD, aside from baseline GFR, proteinuria, low albumin- and hemoglobin-levels and a history of CVD (all p<0.05).
Conclusions
We found hepcidin-25 to be associated with EPO and impaired kidney function in diabetic CKD. Elevated hepcidin-25 and EPO-levels were independent predictors of mortality, while hepcidin-25 was also predictive for progression of CKD. Both hepcidin-25 and EPO may represent important prognostic factors of clinical outcome and have the potential to further define “high risk” populations in CKD.
Background
The maximal expiratory flow at 50 % of the forced vital capacity (MEF50) is the flow where half of forced vital capacity (FVC) remains to be exhaled. A reduced MEF50 has been suggested as a surrogate marker of small airways disease. The diagnostic and prognostic utility of this easy to assess spirometric variable in persons with respiratory symptoms, but without COPD is unclear.
Methods
We used data from the UHFO-COPD cohort in which 405 community-dwelling persons aged 65 years or over, and a general practitioner’s diagnosis of chronic obstructive pulmonary disease (COPD) underwent pulmonary function testing and echocardiography. In total 161 patients had no COPD according to the spirometric GOLD criteria. We considered MEF50 as reduced if < 60 % of predicted.
Results
Of the 161 patients without COPD (mean age 72 ± 5.7 years; 35 % male; follow-up 4.5 ± 1.1 years), 61 (37.9 %) had a reduced MEF50. They were older, had more pack-years of smoking, more respiratory symptoms, and used more frequently inhaled medication than the remaining 100 subjects. A reduced MEF50 was nearly twice as often associated with newly detected heart failure (HF) at assessment (29.5 % vs. 15.6 %, p = 0.045). In age-and sex-adjusted Cox regression analysis, a reduced MEF50 was significantly associated with episodes of acute bronchitis (hazard ratio 2.54 95 % confidence interval (1.26; 5.13) P = 0.009), and in trend with pneumonia (2.14 (0.98; 4.69) P = 0.06) and hospitalizations for pulmonary reasons (2.28 (0.93; 5.62) P = 0.07).
Conclusions
In older community-dwelling persons with pulmonary symptoms but without COPD, a reduced MEF50 may help to uncover unrecognized HF, and identify those at a higher risk for episodes of acute bronchitis, pneumonia and hospitalizations for pulmonary reasons. Echocardiography and close follow-up should be considered in these patients.
Right ventricle (RV) dysfunction is a key outcome determinant and a leading cause of death for patients with chronic thromboembolic pulmonary hypertension (CTEPH). In this report, we followed the 5-year clinical journey of a patient with CTEPH. The tricuspid pressure gradient was significantly increased in the early phase of CTEPH and “normalized” at the late phase of this patient’s clinical journey, but this “normalized” gradient is not a positive treatment response but rather an ominous sign of advancing right heart failure owing to an exhaustion of RV contractile function. Thus, appropriate interpretation of the tricuspid pressure gradient change is of importance for assessing RV dysfunction and treatment outcome during follow-up in patients with CTEPH. Besides systolic pulmonary artery pressure (SPAP), other RV functional parameters such as tricuspid annular plane systolic excursion, RV fractional area change, and RV longitudinal strain, together with clinical markers, may provide additional guidance regarding functional improvement or progression in patients with CTEPH.
Background
Comprehensive evidence on the incidence, time course and independent risk factors of metachronous peritoneal carcinomatosis (metaPC) in gastric cancer patients treated with curative intent in the context of available systemic combination chemotherapies is lacking.
Methods
Data from a prospectively collected single-institutional Center Cancer Registry with 1108 consecutive patients with gastric adenocarcinoma (GC), clinical, histological and survival data were analyzed for independent risk factors and prognosis with focus on the development of metaPC. Findings were then stratified to the time periods of treatment with surgery alone, 5-Fluorouracil-only and contemporary combined systemic perioperative chemotherapy strategies, respectively.
Results
Despite R0 D2 gastrectomy (n = 560), 49.6% (±5.4%) of the patients were diagnosed with tumour recurrence and 15.5% (±1.8%) developed metaPC after a median time of 17.7 (15.1-20.3) months after surgery resulting in a tumour related mortality of 100% with a median survival of 3.0 months (2.1 – 4.0). Independent risk factors for the development of metaPC were serosa positive T-category, nodal positive-status, signet cell and undifferentiated gradings (G3/G4). Contemporary systemic combination chemotherapy did not improve the incidence and prognosis of metaPC (p = 0.54).
Conclusions
Despite significant improvements in the overall survival for the complete cohort with gastric cancer over time, those patients with metaPC did not experience the same benefits. The lack of change in the incidence, and persistent poor prognosis of metaPC after curative surgery expose the need for further prevention and/or improved treatment options for this devastating condition.
Thalidomid als Therapieoption beim fortgeschrittenen Nebennierenkarzinom: Eine retrospektive Studie
Das adrenokortikale Karzinom (ACC) ist ein seltener Tumor mit einer schlechten Prognose. Im fortgeschrittenen Stadium gelten Mitotane und zytotoxische Chemotherapien als Standardtherapie, mit denen allerdings nur kurzzeitig eine Tumorkontrolle erreicht werden kann. Daher machte Thalidomid Hoffnung auf eine mögliche ´Rettungs´-Therapie.
Im Rahmen dieser retrospektiven Studie sollte der Nutzen und die Tolerabilität von Thalidomid beim fortgeschrittenen Nebennierenkarzinom untersucht werden. Insgesamt konnten 15 Patienten aus dem deutschen Nebennnierenkarzinomregister herausgefiltert werden, die den Einschlusskriterien entsprachen und Thalidomid als off-label erhalten haben.
Als Endpunkt wurden das progressionsfreie Überleben, ausgewertet geblinded gemäß RECIST 1.1., und das Gesamtüberleben festgelegt.
Alle 15 Patienten (7 Männer; medianes Alter 48,9 (Range 34,4 – 69,0) Jahre) waren bereits mit bis zu sechs systemischen Therapien vorbehandelt. Thalidomid wurde in einer Dosierung gemäß Verträglichkeit verabreicht (mediane Startdosis 100 mg/d) und das Restaging erfolgte alle 12 Wochen, das Erste im Median nach 10,9 Wochen.
Das progressionfreie Überleben lag im Median bei 11,1 Wochen (Range 4,4 – 34,4 Wochen), das Gesamtüberleben lag im Median bei 34,4 Wochen (Range 5,1 – 111,1 Wochen).
Während der erste Patient, der eine Krankheitsstabilisierung erfahren hat, die Behandlung aufgrund von Epistaxis und Diarrhoe Grad I nach 22,3 Wochen abbrach, zeigte der zweite Patient nach 34,4 Wochen weiterhin eine Krankheitsstabilisierung, obwohl er unter den vorangegangenen vier zytotoxischen Therapien progredient war.
Unter Thalidomid wurden nur geringgradige bis mäßige Nebenwirkungen beobachtet (hauptsächlich Fatigue und gastrointestinale Nebenwirkungen).
Schlussfolgerung: Thalidomid ist ein gut verträgliches Medikament, das nur bei einer Minderheit zahlreich vortherapierter Patienten zu einer Krankheitsstabilisierung führte.
Background: Accurate preoperative assessment of the aortic annulus dimension is crucial for successful transcatheter aortic valve implantation (TAVI). In this study we examined the accuracy of a novel method using two-dimensional transesophageal echocardiography (2D-TEE) for measurement of the aortic annulus.
Methods: We evaluated the theoretical impact of the measurement of the annulus diameter and area using the circumcircle of a triangle method on the decision to perform the procedure and choice of the prosthesis size. Results: Sixty-three consecutive patients were scheduled for TAVI. Mean age was 82 +/- 4 years, and 25 patients (55.6 %) were female. Mean aortic annulus diameter was 20.3 +/- 2.2 mm assessed by TEE on the mid-esophageal long-axis view and 23.9 +/- 2.3 mm using CT (p < 0.001). There was a tendency for the TEE derived areas using the new method to be higher (p < 0.001). The TEE measurements were on average 42.33 mm(2) higher than the CT measurements without an evidence of a systematic over-or under-sizing (p = 1.00). Agreement between TEE and CT chosen valve sizes was good overall (kappa = 0.67 and weighted kappa = 0.71). For patients who turned out to have no AR, the two methods agreed in 84.6 % of patients.
Conclusions: CT remanis the gold standard in sizing of the aortic valve annulus. Nevertheless, sizing of the aortic valve annulus using TEE derived area may be helpful. The impact of integration of this method in the algorithm of aortic annulus sizing on the outcome of patients undergoing TAVI should be examined in future studies.
Mineralocorticoid receptor (MR) inactivation in mice results in early postnatal lethality. Therefore we generated mice in which MR expression can be silenced during adulthood by administration of doxycycline (Dox). Using a lentiviral approach, we obtained two lines of transgenic mice harboring a construct that allows for regulatable MR inactivation by RNAi and concomitant expression of eGFP. MR mRNA levels in heart and kidney of inducible MR knock-down mice were unaltered in the absence of Dox, confirming the tightness of the system. In contrast, two weeks after Dox administration MR expression was significantly diminished in a variety of tissues. In the kidney, this resulted in lower mRNA levels of selected target genes, which was accompanied by strongly increased serum aldosterone and plasma renin levels as well as by elevated sodium excretion. In the healthy heart, gene expression and the amount of collagen were unchanged despite MR levels being significantly reduced. After transverse aortic constriction, however, cardiac hypertrophy and progressive heart failure were attenuated by MR silencing, fibrosis was unaffected and mRNA levels of a subset of genes reduced. Taken together, we believe that this mouse model is a useful tool to investigate the role of the MR in pathophysiological processes.
Background: Nontraumatic osteonecrosis of the femoral head (NONFH) is a debilitating disease that represents a significant financial burden for both individuals and healthcare systems. Despite its significance, however, its prevalence in the Chinese general population remains unknown. This study aimed to investigate the prevalence of NONFH and its associated risk factors in the Chinese population.
Methods: A nationally representative survey of 30,030 respondents was undertaken from June 2012 to August 2013. All participants underwent a questionnaire investigation, physical examination of hip, and bilateral hip joint X-ray and/or magnetic resonance imaging examination. Blood samples were taken after overnight fasting to test serum total cholesterol, triglyceride, and high-density lipoprotein (HDL) and low-density lipoprotein (LDL) levels. We then used multivariate logistic regression analysis to investigate the associations between various metabolic, demographic, and lifestyle-related variables and NONFH.
Results: NONFH was diagnosed in 218 subjects (0.725%) and the estimated NONFH cases were 8.12 million among Chinese people aged 15 years and over. The prevalence of NONFH was significantly higher in males than in females (1.02% vs. 0.51%, \(\chi^2\) = 24.997, P < 0.001). Among NONFH patients, North residents were subjected to higher prevalence of NONFH than that of South residents (0.85% vs. 0.61%, \(\chi^2\) = 5.847, P = 0.016). Our multivariate regression analysis showed that high blood levels of triglycerides, total cholesterol, LDL-cholesterol, and non-HDL-cholesterol, male, urban residence, family history of osteonecrosis of the femoral head, heavy smoking, alcohol abuse and glucocorticoid intake, overweight, and obesity were all significantly associated with an increased risk of NONFH.
Conclusions: Our findings highlight that NONFH is a significant public health challenge in China and underscore the need for policy measures on the national level. Furthermore, NONFH shares a number of risk factors with atherosclerosis.
Tyrosinkinaseinhibitoren nehmen in der modernen Onkologie einen wachsenden Stellenwert ein. Sunitinib wirkt als Multityrosinkinaseinhibitor einerseits antiangiogenetisch, andererseits auch direkt antiproliferativ auf Tumorzellen. Im Tierversuch sind unter Sunitinib adrenotoxische Wirkungen beschrieben. Für das Nebennierenkarzinom, eine sehr seltene Tumorerkrankung mit schlechter Prognose, werden dringend neue Therapieoptionen benötigt. In dieser Arbeit wurde der Effekt von Sunitinib auf die Proliferation von Nebennierenkarzinomzellen in vitro und auf deren Steroidbiosynthese untersucht.
Es konnte gezeigt werden, dass Sunitinib dosisabhängig auf die beiden Nebennierenkarzinomzelllinien NCI-h295(R) und SW-13 antiproliferativ wirkt (SW-13: unter 0,1 µM Sunitinib 96 ± 7 %; 1 µM 90 ± 9 %*; 5 µM 62 ± 6 %*, Kontrollen 100 ± 9 %, ab 1 µM p<0,05). Steroidanalysen in den Zellkulturüberständen von NCI-h295-Zellen mittels Isotopenverdünnungs-/Gaschromatographie-Massenspektrometrie belegen eine Abnahme der Cortisolsekretion (1 μM 90,1 ± 1,5 %*, 5 μM 57,2 ± 0,3 %*, Kontrollen 100 ± 2,4 %), während bestimmte Vorläuferhormone akkumulieren. Der beobachtete Anstieg der Quotienten von 17-OH-Pregnenolon zu 17-OH-Progesteron und DHEA zu Androstendion belegt eine partielle Hemmung der Steroidsynthese auf Ebene der 3ß-Hydroxysteroiddehydrogenase (HSD3B2). Nachdem eine direkte Hemmung des Enzyms HSD3B2 mittels Hefe-Mikrosomen-Assay ausgeschlossen werden konnte, bestätigte sich auf RNA- mittels Real-Time-PCR und Proteinebene mittels Western Blot eine dosisabhängige Hemmung der Transkription und Translation des Enzyms (mRNA: 1 μM 47 ± 7 %*; 5 μM 33 ± 7 %*; 10 μM 27 ± 6 %*; Protein: 1 μM 82 ± 8 %; 5 μM 63 ± 8 %*; 10 μM 55 ± 9 %*). Auch für CYP11B1 zeigte sich eine dosisabhängige Transkriptionshemmung durch Sunitinib, andere Enzyme wie CYP11A1 dagegen werden nicht beeinflusst.
Wenn sich diese in vitro Effekte bei Patienten unter Sunitinib-Therapie bestätigen sollten, könnte es bei einzelnen Patienten zu einer klinisch relevanten Nebenniereninsuffizienz kommen. Eine eindeutige Wirksamkeit von Sunitinib als Therapieoption beim Nebennierenkarzinom konnte im Rahmen der SIRAC-Studie nicht bestätigt werden. Hier ist jedoch anzumerken, dass wahrscheinlich eine gravierende Medikamenteninteraktion mit Mitotane zu einer Reduktion des Effekts von Sunitinib beigetragen hat.
Ziel:
In dieser Arbeit wurden die Auswirkung der milden und moderaten Hyponatriämie (125-133 mmol/l) auf das Befinden der betroffenen Patienten im Hinblick auf neurokognitive Funktion und klinische Symptomatik untersucht. In mehreren Studien wurde über eine erhöhte Sturzneigung sowie eine Minderung der Konzentrationsfähigkeit bei einem nur leicht erniedrigten Serumnatriumspiegel berichtet.
Methoden:
Die Testungen fanden im Longitudinalvergleich mit jeweils den gleichen Patienten vor und nach Anhebung des Serumnatriumspiegels statt, sodass bis auf das Serumnatrium keine Beeinflussung gegeben war. Die Patienten waren im Durchschnitt 61 Jahre alt und besaßen einen durchschnittlichen Serumnatriumwert von 128,7 mmol/l; die Ätiologie der Hyponatriämie war heterogen (normovolämische und hypervolämische Hyponatriämie).
Um die Aufmerksamkeitsdefizite aufzudecken, fanden Testungen der Patienten (n=16) mit vier verschiedenen Untertests der TAP, einem Standardprogramm der Psychologie, statt: „Alertness“ mit „phasischer Alertness“, „Daueraufmerksamkeit“, „Geteilte Aufmerksamkeit“ und „Go/Nogo“. Um die Konzentrationsfähigkeit gegen eine Störung wie Stress beurteilen zu können, wurde die „Wiener Form A“, ein Subtyp des Wiener Testsystems verwendet, ein ebenfalls in der Psychologie genutzter Test (n=12).
Verwendet wurde außerdem ein selbst designter Fragebogen (n=18), der auf die in der Literatur berichteten Symptome einer leichten Hyponatriämie zugeschnitten war. Ein und dieselben Patienten wurden jeweils vor und nach Anhebung des Serumnatriumwertes getestet.
Ergebnisse:
Es zeigte sich, dass bei verschiedenen klinischen Symptomen wie Krämpfen (p= 0,018) und Gangunsicherheit (p= 0,092) signifikante Verbesserungen gefunden wurden. Auch bei dem unspezifischeren Symptom Müdigkeit (p= 0,04) konnte eine Tendenz zur Besserung nach Anhebung des Serumnatriumspiegels verzeichnet werden. In den Aufmerksamkeitstests war – im Gegensatz zu einer Vorstudie, in der ebenfalls die TAP zur Beurteilung der neurokognitiven Funktion herangezogen worden war - weder die „Alertness“, also die Reaktionsgeschwindigkeit, noch die Reaktions-Selektionsleistung beim Test „Go/Nogo“ signifikant alteriert. Auch die „Daueraufmerksamkeit“ und „Geteilte Aufmerksamkeit“ wurden nicht signifikant beeinflusst.
Ebenso wenig wie die verschiedenen Aufmerksamkeitfunktionen beeinflusst werden, ist die Reaktionsfähigkeit unter Stress wesentlich herabgesetzt. Denn der Stressreiz im Test „Wiener Form A“ führte bei den Patienten in Hyponatriämie zu keinem signifikant schlechteren Ergebnis als der gleiche Test mit denselben Patienten bei angehobenem Serumnatriumwert.
Zusammefassung:
Zusammenfassend kann aus der vorliegenden Studie gefolgert werden, dass eine milde und moderate Hyponatriämie keine Auswirkungen auf die neurokognitive Funktion hat und insofern die Sturzneigung bei älteren Patienten wohl nicht beeinflusst. Bei bestimmten klinischen Symptomen konnte eine teilweise signifikante Verbesserung verzeichnet werden, jedoch gibt es zu viele mögliche Einflussfaktoren, als dass man eine endgültige Aussage treffen könnte. Die Ergebnisse aus Vorstudien müssen in Frage gestellt werden. Um den tatsächlichen Einfluss einer milden und moderaten Hyponatriämie auf die Aufmerksamkeitsfunktion zu klären sowie die Beeinträchtigung bei bestimmten klinischen Symptomen beweisen zu können, bedarf es weiterer Studien in randomisiertem Doppelblinddesign mit höherer Patientenzahl.
