Introduction: Although there has been a worldwide emergence and spread of methicillin-resistant Staphylococcus aureus (MRSA), little is known about the molecular epidemiology of MRSA in Tanzania.
Methodology: In this study, we characterized MRSA strains isolated from clinical specimens at the Bugando Medical Centre, Tanzania, between January and December 2008. Of 160 S. aureus isolates from 600 clinical specimens, 24 (15%) were found to be MRSA. Besides molecular screening for the Panton Valentine leukocidin (PVL) genes by PCR, MRSA strains were further characterized by Multi-Locus Sequence Typing (MLST) and spa typing.
Results: Despite considerable genetic diversity, the spa types t690 (29.1%) and t7231 (41.6%), as well as the sequence types (ST) 88 (54.2%) and 1797 (29.1%), were dominant among clinical isolates. The PVL genes were detected in 4 isolates; of these, 3 were found in ST 88 and one in ST1820. Resistance to erythromycin, clindamicin, gentamicin, tetracycline and co-trimoxazole was found in 45.8%, 62.5%, 41.6%, 45.8% and 50% of the strains, respectively.
Conclusion: We present the first thorough typing of MRSA at a Tanzanian hospital. Despite considerable genetic diversity, ST88 was dominant among clinical isolates at the Bugando Medical Centre. Active and standardized surveillance of nosocomial MRSA infection should be conducted in the future to analyse the infection and transmission rates and implement effective control measures.
A total of 36 Escherichia coli urinary tract isolates (UTI) of serotype 06, with different combinations of capsule ( K) and flagellin ( H) antigens, were analysed according to the outer membrane pattern (OMP), serum resistance properties, mannose-resistant hemagglutination using various types of erythrocytes, and also for the genetic presence and the expression of Pfimbriae. S fimbriae/F1 C fimbriae, Type 1 fimbriae, aerobactin and hemolysin. Twenty selected strains were further analysed by pulsed field gel electrophoresis (PFGE), elaborating genomic profilas by Xba I cleavage and subsequent Southern hybridization to virulence-associated DNA probes. lt could be shown that 06 UTI isolates represent a highly heterogeneaus group of strains according to the occurrence and combination of these traits. Relatedness an the genetic and the phenotypic Ievei was found for some of the strains exhibiting the same 0: K: H: F serotype. DNA Iang-range mapping further indicated some interesting features, according to the copy number and the genomic linkage of virulence genes.
Hintergrund: Infektionen mit dem Respiratory Syncytial Virus (RSV) sind die häufigste virale Ursache für respiratorische Erkrankungen bei Säuglingen und Kleinkindern. Reinfektionen treten lebenslang auf. Es wurden zwei Typen (A und B) und mehrere Genotypen beschrieben. Die vorliegenden Daten über die molekulare Epidemiologie von RSV in Deutschland sind nur begrenzt. Material und Methoden: Zwischen Januar 2002 und Juli 2006 wurden 221 Nasenrachensekrete (NRS) von Kindern, welche in der Universitätskinderklinik Würzburg behandelt wurden, durch Routine-Untersuchung mit einem Immunfluoreszenztest auf RSV-Antigen positiv befunden. Die phylogenetische Analyse wurde aus Restmaterial von 211 NRS durchgeführt, indem die zweite variable Region des G-Gens amplifiziert und sequenziert wurde. Ergebnisse: Insgesamt war die Prävalenz von Typ A-Viren mit 69,5 % größer als die der Typ B-Viren mit 30,5 %. RSV Typ A war das dominierende Virus in allen Saisons außer in der Saison 2002-2003. Über den gesamten Beobachtungszeitraum traten drei A-Genotypen (GA2, GA5 und GA7) und vier B-Genotypen (GB3, SAB3, BA und ein neuer Genotyp) auf. Die Genotypen GA2, GA5, SAB3 und BA waren am häufigsten im Umlauf und in beinahe allen Saisons prävalent. Unter den B-Genotypen nahm der Anteil des Genotyps BA von 25 % (2002) auf 92 % (2005-2006) zu. Drei Typ B-Sequenzen wurden einem neuen Genotyp zugeordnet, welcher BWUE benannt wurde. Es wurde eine Reinfektion mit demselben Genotyp (GA5) bei einem Kind beobachtet, welches im Alter von 12 und 28 Monaten mit einer RSV-Infektion hospitalisiert war. Schlußfolgerung: Die Ergebnisse unserer Studie stehen in Einklang mit der molekularen Epidemiologie von RSV in anderen geographischen Regionen. Wir beobachteten sowohl Genotypen, welche über mehrere Saisons prävalent waren, als auch Genotypen, welche über den beobachteten Zeitraum zunehmend dominanter werdend andere Genotypen verdrängten. Zudem wurde ein neuer B-Genotyp entdeckt.
