Die vorliegende Arbeit überprüft an einem nach Alter, Geschlecht, Barthel-Index und Mini-Mental-State-Test gematchten geriatrischen Patientenkollektiv mit erstmaligem Schlaganfall die Wirksamkeit einer vorausgegangenen Akutbehandlung an einer Stroke Unit (n=59) gegenüber einer allgemeinen (internistischen oder neurologischen) stationären Akutbehandlung (n=59) für die Prognose im Laufe einer nachfolgenden geriatrischen Rehabilitationsbehandlung. Hintergrund dieser Frage ist der erhöhte ökonomische Druck im Gesundheitswesen, der eine Effizienzprüfung einer personell, technisch und logistisch aufwändigeren und damit teureren Behandlung auf einer Spezialstation verlangt. Bei Anwendung zahlreicher funktioneller Skalen und Erhebung einiger sozioökonomischer Faktoren zeigte sich auf Signifikanzniveau, dass die auf Stroke Unit Vorbehandelten bei Aufnahme in die Rehabilitation motorisch schwerer beeinträchtigt waren (timed up and go-Test p=0,044, Lachs-Test p=0,34) und sich dann ausgeprägter (Transferleistung p=0,024) auf ein bei Rehabilitationsende schließlich vergleichbares Leistungsniveau verbesserten. Die ursprünglich geplante Langzeiteffizienzbetrachtung im Gruppenvergleich scheiterte an Datenschutzbedenken. Gesundheitsökonomisch relevant ist, dass die Vorverweildauer im Akutkrankenhaus bei Stroke Unit-Patienten sechs Tage kürzer war, die Rehabilitationsdauer allerdings vier Tage länger. Weitergehende Kostenbetrachtungen scheiterten am Unwillen zur Leistungsoffenlegung verschiedener Beteiligter im Gesundheitssystem. Eine plausible Erklärung für diese positive motorische Leistungsweiterentwicklung nach Stroke Unit-Vorbehandlung kann in einer frühzeitigeren und effektiveren Anstrengung durch Krankengymnastik, Ergotherapie, Logopädie, aktivierende Pflege, „enriched environment“ gesucht werden, die sich positiv auf die Plastizität im Gehirn als wesentliche Bedingung zur Funktionswiedergewinnung auswirken könnte, was aber noch umstritten ist und Ziel weiterer Untersuchungen sein muss.
Ziel dieser Arbeit war es, die Auswirkung einer chronischen Wunde auf die Lebensqualität von Diabetikern zu untersuchen. Dazu sollte die Lebensqualität von Diabetikern ohne chronische Wunde mit der Lebensqualität von Diabetikern mit chronischer Wunde verglichen werden. Analog dazu erfolgte ein Vergleich von Patienten mit und ohne chronischer Wunde, aber zusätzlicher dialysepflichtiger Niereninsuffizienz. Befragt wurden insgesammt 215 Patienten. Bei Patienten mit chronischer Wunde erfolgten jeweils drei Befragungen um eventuelle Veränderungen der Lebensqualität im Therapieverlauf zu ermittlen. Die Interviews erfolgten zum Zeitpunkt der Erstbefragung sowie nach einem und drei Monaten. Gemessen wurde die Lebensqualität durch die krankheitsübergreifenden Messinstrumente "Short-Form-36" (SF-36), dem "Nottingham Health Profile" (NHP)sowie dem neu entwickelten, krankheitsspezifischen "Würzburger Wundscore". Es zeigte sich, dass schon das Bestehen eines Diabetes mellitus zu einer erheblichen Verschlechterung der Lebensqualität führt. Durch eine chronische Wunde kommt es ebenfalls zu einer deutlichen Verschlechterung der Lebensqualität, vor allem sind hier physische Bereiche stark eingeschränkt. Durch eine Dialysepflichtigkeit wird die Lebensqualität zwar auch eingeschränkt, jedoch nicht im selben Umfang wie durch eine chronische Wunde bzw. durch das alleinige Bestehen eines Diabetes mellitus. Bei Patienten mit chronischer Wunde wurde bei Untersuchungen im Therapieverlauf zwischen einem günstigen und einem ungünstigen Verlauf unterschieden. Es konnte hier keine signifikanten Veränderungen der Lebensqualität im Verlauf gezeigt werden. Es zeigte sich, dass eine gute Lebensqualität vor allem bei physischen Komponenten negativ mit dem Alter der Befragten korreliert. Männliche Diabetiker mit chronischer Wunde haben in fast allen Bereichen eine bessere Lebensqualität als Frauen. Der "Würzburger Wundscore" erwies sich als ein valides Instrument zur Messung der Lebensqualität bei Diabetikern mit chronischer Wunde mit einer höheren Änderungssensitivität bei Veränderungen der Lebensqualität bei günstigem Heilungsverlauf im Vergleich zu den SF-36 und NHP.
Informationen über die Landbedeckung und die mit der anthropogenen Komponente verbundenen Landnutzung sind elementare Bestandteile für viele Bereiche der Politik, Wirtschaft und Wissenschaft. Darunter fallen beispielsweise die Strukurentwicklungsprogramme der EU, die Schadensregulierung im Versicherungswesen und die Modellierung von Stoffkreisläufen. CORINE Land Cover (CLC) wurde infolge eines erweiterten Bedarfs an einem europaweit harmonisierten Datensatz der Landoberfläche erstellt. Das CORINE Projekt weist für diese Arbeit eine hohe Relevanz durch die regelmäßigen Aktualisierungen von 10 Jahren, dem Einsatz der Daten in vielen europäischen und nationalen Institutionen und der guten Dokumentation der CORINE Nomenklatur auf. Die Erstellung der Daten basiert auf der computergestützten manuellen Interpretation, da automatische Verfahren durch die Komplexität der Aufgabenstellung und Thematik nicht in der Lage waren, den menschlichen Interpreten zu ersetzen. Diese Arbeit stellt eine Methodik vor, um CORINE Land Cover aus optischen Fernerkundungsdaten für eine kommende Aktualisierung abzuleiten. Hierzu dienen die Daten von CLC 1990 und der Fernerkundungsdatensatz Image 2000 als Grundlage, sowie die CLC 2000 Klassifikation als Referenz. Die entwickelte und in dem Softwarepaket gnosis implementierte Methodik wendet die objektorientierte Klassifikation in Kombination mit Theorien aus der menschlichen Bildwahrnehmung an. In diesen Theorien wird die Bildwahrnehmung als informationstechnischer Prozess gesehen, der den Klassifikationsprozess in die drei folgenden Subprozesse unterteilt: Bildsegmentierung, Merkmalsgenerierung und Klassenzuweisung. Die Bildsegmentierung generiert aus den untersten Bildprimitiven (Pixeln) bedeutungsvolle Bildsegmente. Diesen Bildsegmenten wird eine Anzahl von bildinvarianten Merkmalen aus den Fernerkundungsdaten für die Bestimmung der CLC Klasse zugewiesen. Dabei liegt die wichtigste Information in der Ableitung der Landbedeckung durch den überwachten Stützvektor-Klassifikator. Die Landoberfläche wird hierzu in zehn Basisklassen untergliedert, um weiteren Merkmalen einen semantischen Unterbau zu geben. Zur Bestimmung der anthropogenen Komponente von ausgewählten Landnutzungsklassen, wie beispielsweise Ackerland und Grünland, wird der phänologische Verlauf der Vegetation durch die Parameter temporale Variabilität und temporale Intensität beschrieben. Neben dem jahreszeitlichen Verlauf der Vegetation können Nachbarschaftsbeziehungen untersucht werden, um weitere anthropogene Klassen und heterogen aufgebaute Sammelklassen beschreiben zu können. Der Versiegelungsgrad als Beispiel für eine Reihe von unscharfen Merkmalen dient der weiteren Differenzierung der verschiedenen Siedlungsklassen aus CORINE LC. Mit Hilfe dieser Merkmale werden die CLC Klassen in abstrakter Form im Klassenkatalog (a-priori Wissensbasis) als Protoklassen beschrieben. Die eigentliche Objekterkennung basiert auf der Repräsentation der CORINE Objekte durch ihre einzelnen Bestandteile und vergleicht die gefundenen Strukturen mit der Wissensbasis. Semantisch homogen aufgebaute Klassen, wie Wälder und Siedlungen oder Protoklassen mit eindeutigen Merkmalen, beispielsweise zur Bestimmung von Grünland durch die Phänologie, können durch den bottom–up Ansatz identifiziert werden. Das übergeordnete CLC Objekt kann direkt aus den Bestandteilen zusammengebaut und einer Klasse zugewiesen werden. Semantisch heterogene Klassen, wie zum Beispiel bestimmte Sammelklassen (Komplexe Parzellenstrukur), können durch ihre Bestandteile validiert werden, indem die Bestandteile eines existierenden CLC Objektes mit der Wissensbasis auf Konsistenz untersucht werden (top–down Ansatz). Eine a-priori Datengrundlage ist für die Erkennung dieser Klassen essentiell. Die Untersuchung der drei Testgebiete (Frankfurt, Berlin, Oldenburg) zeigte, dass von der CORINE LC Nomenklatur 13 Klassen identifiziert und weiteren 14 Klassen validiert werden können. Zehn Klassen können durch diese Methodik aufgrund fehlender Merkmale oder Zusatzdaten nicht klassifiziert werden. Die Gesamtgenauigkeit der automatisierten Klassifikation für die Testgebiete beträgt zwischen 70% und 80% für die umgesetzten Klassen. Betrachtet man davon einzelne Klassen, wie Siedlungs-, Wald- oder Wasserklassen, wird aufgrund der verwendeten Merkmale eine Klassifikationsgenauigkeit von über 90% erreicht. Ein möglicher Einsatz der entwickelten Software gnosis liegt in der Unterstützung einer kommenden CORINE Aktualisierung durch die Prozessierung der identifizierbaren Klassen. Diese CLC Klassen müssen vom Interpreten nicht mehr überprüft werden. Für bestimmte CLC Klassen aus dem Top-down Ansatz wird der Interpret die letzte Entscheidung aus einer Auswahl von Klassen treffen müssen. Weiterhin können die berechneten Merkmale, wie die temporalen Eigenschaften und der Versiegelungsgrad dem Bearbeiter als Entscheidungsgrundlage zur Verfügung gestellt werden. Der Einsatz dieser neu entwickelten Methode führt zu einer Optimierung des bestehenden Aufnahmeverfahrens durch die Integration von semi-automatisierten Prozessen.
Das Ficksche Prinzip gilt als der Goldstandard der Bestimmung des Herzminutenvolumens. Neben der invasiven Bestimmung über einen Pulmonaliskatheter steht die nicht-invasive Rückatmungsmethode (Rebreahting) zur Verfügung. Bereits 1996 ist von FW Schardt eine weitere nicht-invasive ergospirometrische Variante vorgestellt worden, die ohne Rückatmung auskommt. Hier wird der gemischt-venösen Kohlendioxidgehalt basierend auf den Kohlendioxidverbrauch berechnet. Ziel der Arbeit ist eine vergleichende Darstellung der Bestimmungsmethode nach Schardt mit dem als Referenzmethode dienenden Rückatmungsverfahren. Besonderes Augenmerk wird auf Messungen unter hohen Belastungen gelegt, denn hier neigt das Rebreathingverfahren zur Unterschätzung des Herzminutenvolumens, zudem wird die Rückatmung selbst für den Patienten unerträglich. Parallel wurde bei 36 Probanden das Herzminutenvolumen sowohl durch Rebreathing (Qt) wie auch nach Schardt (HMV) ergospirometrisch auf dem Fahrradergometer (in 50 Watt Stufen) bis maximal 350 Watt bestimmt. Ausgewertet wurde die absolute bzw. prozentuale Abweichung des HMV von Qt, sowie HMV vom Mittelwert beider Methoden nach Bland/Altman bzw. Critchley/Critchley. Unter Einbeziehung der Ergebnisse aller Belastungsstufen wird deutlich, dass die Werte für HMV bei niedrigeren Ausgangswerten unter den Werten der Rückatmungsmethode (Qt) liegen. Dagegen werden bei hohen Belastungen für HMV höhere Werte berechnet als für Qt. Die größten Abweichungen sind in Ruhe die zu erkennen. Die Abweichung des HMV vom Mittelwert beider Methoden liegt in Ruhe im Mittel bei 18,8%, unter niedriger Belastung bei 8,7-9,4%. Die Grenzen der Übereinstimmung überschreiten jedoch die Grenzen der Genauigkeit. Die niedrigsten Abweichungen sind bei mittelschwerer Belastung (150-200 Watt) zu verzeichnen, sowie geringe Abweichungen unter sehr hoher Belastung (1,2-4,0%). Die Grenzen der Übereinstimmung liegen innerhalb der Grenzen der Genauigkeit. Die prozentuale Abweichung der Ergebnisse für das Herzminutenvolumen nach Schardt vom Mittelwert beider Methoden liegt über allen Belastungsstufen innerhalb der von Critchley und Critchley geforderten +/- 20 Prozent. Insgesamt konnte gezeigt werden, dass die Methode nach Schardt eine Variante des Fickschen Prinzips liefert, die als einfache, nicht invasive Maßnahme auch unter hoher Belastung und bei wiederholten Messungen anwendbar ist und insbesondere unter mittleren und hohen Belastungen mit der Rebreathingmethode austauschbar ist.
Ziel dieser Arbeit war die Untersuchung der psychischen Befindlichkeit und anderer gesundheitsbezogenen Konditionen der Frauen und Männer mit familiären Mamma- und Ovarialkarzinomrisiko sowie die Klärung hinsichtlich der Bewältigung und Auswirkung genetischer Risikoinformation. Es wurden Risikowahrnehmung, Informationsstand, Inanspruchnahme der Beratungsangebote sowie der Früherkennungsmaßnahmen, Einstellung gegenüber genetischer Brustkrebsdiagnostik und familiärer/sozialer Kommunikation untersucht. Die vollständig ausgefüllten Fragebögen von Ratsuchenden und Betroffenen, die an der Beratung und Befragung im Zentrum für „Familiären Brust-/Eierstockkrebs“ teilgenommen haben, wurden von uns ausgewertet. Für die beratenden Institutionen ist das Wissen der vielfältigen psychischen und sozialen Folgen bei den Testsuchenden und deren Familien sehr wichtig. Nur so kann das Betreuungskonzept und das Beratungsangebot verbessert werden.
Meningokokken sind nach wie vor eine wichtige Ursache für Gehirnhautentzündungen und Sepsen weltweit, vor allem bei Kindern und Jugendlichen. Weil viele Pathomechanismen dieses Erregers bislang noch unvollständig verstanden sind, wurden im Rahmen der vorliegenden Doktorarbeit populationsbiologische und pathogenetische Aspekte von Neisseria meningitidis untersucht. Die Kapsel ist der hauptsächliche Pathogenitätsfaktor von Meningokokken und wichtig für die Besiedelung von neuen Wirten. Isolate von symptomfreien Trägern sind allerdings häufig unbekapselt. Um Ursachen oder Mechanismen für den Verlust der Kapselexpression aufzudecken, wurden insgesamt 166 Isolate der Bayerischen Meningokokkenträgerstudie untersucht. Alle Isolate besaßen sämtliche zur Kapselsynthese notwendigen Gene, exprimierten aber keine Kapsel. Bei 39 Isolaten fanden sich Längenvariationen in homopolymeren Sequenzen (slipped strand mispairing, SSM) in den Genen siaA und siaD. 46 Isolate enthielten Insertionselemente (IS1301, IS1016 und IS1106) in den Genen der Kapselsynthese. Irreversible Mutationen (Deletionen, Insertionen, Basensubstitutionen) wurden bei 47 Isolaten gefunden. Veränderungen der Promotorregion schienen keine Rolle zu spielen. Es wurden bei insgesamt sechs Isolaten zwei nicht-synonyme Mutationen in unmittelbarer Nähe zum putativen aktiven Zentrum der UDP-N- Acetylglukosamin-2-Epimerase entdeckt, die einen Verlust der Kapselsynthese erklären könnten. Insgesamt wurden keine Akkumulationen von Mutationen in defekten Genen gefunden und es gab auch keine Korrelationen zwischen den verschieden Ursachen und bestimmten klonalen Linien. Die erhaltenen Ergebnisse legen nahe, dass die meisten der zur Blockierung der Kapselexpression führenden Ereignisse erst im aktuellen Wirt aufgetreten sind und dass zumindest bei bestimmten klonalen Linien die Verbreitung von der Expression einer Kapsel abhängig ist. Viele pathogene Bakterien nutzen zur Infektion des Menschen die ubiquitär im Körper vorkommende Protease Plasmin. Dazu binden diese Plasmin oder das Proenzym Plasminogen. Auch Meningokokken interagieren mit Plasmin und Plasminogen. Im Rahmen der vorliegenden Arbeit konnten drei Rezeptormoleküle für Plasminogen identifiziert werden. Die drei Proteine Enolase, DnaK und Peroxiredoxin konnten mit verschiedenen Methoden auf der Oberfläche der Erreger nachgewiesen werden. Die Bindung des Plasminogens ist bei Meningokokken ausschließlich über Lysinreste der Rezeptoren vermittelt, die C-terminalen Lysinreste der hier identifizierten Rezeptormoleküle spielen aber, wenn überhaupt, nur eine untergeordnete Rolle. Die Bindung von Plasminogen war durch rekombinante Rezeptorproteine konzentrationsabhängig inhibierbar. Plasminogen konnte von Meningokokken auch aus dem Serum rekrutiert werden. Gebundenes Plasminogen war mit uPA (Urokinase Plasminogen Aktivator) aktivierbar und physiologisch aktiv, was durch die Degradation von Fibrinogen nachgewiesen wurde. Das gebundene Plasmin wurde durch die Bakterien vor der Desaktivierung durch α2- Antiplasmin geschützt. Des Weiteren konnte gezeigt werden, dass Meningokokken auch mit weiteren Faktoren des Fibrinolyse-Systems (uPA) interagieren. Sie rekrutierten uPA an ihre Oberfläche und gebundenes uPA war physiologisch aktiv. Die erhaltenen Ergebnisse bestärken die These, dass Meningokokken die Faktoren des Fibrinolyse-Systems für ihre Pathogenese nutzen.
The live sciences currently undergo a paradigm shift to computer aided discoveries. Discoveries in the live sciences were historically made by either direct observation or as a result of chemical assays. Today we see a growing shift toward computer aided analysis and visualization. This gradual process happens in microscopy. Multidimensional laser scanning microscopy can acquire very complex multichannel data from fixed or live specimen. New probes such as visible fluorescent proteins let us observe the expression of genes and track protein localization. Ion sensitive dyes change intensity with the concentration of ions in the cell. The laser scanning confocal allows us to record these processes in three dimensions over time. This work demonstrates the application of software analysis to multidimensional microscopy data. We introduce methods for volume investigation, ion flux analysis and molecular modeling. The visualization methods are based on a multidimensional data model to accommodate complex datasets. The software uses vector processing and multiple processors to accelerate volume rendering and achieve interactive rendering. The algorithms are based on human visual perception and allow the observer a wide range of mixed render modes. The software was used to reconstruct the pituitary development in zebrafish and observe the degeneration of neurons after injury in a mouse model. Calicum indicator dyes have long been used to study calcium fluxes. We optimized the imaging method to minimize impact on the cell. Live cells were imaged continuously for 45 minutes and subjected to increasing does of a drug. We correlated the amplitude of calcium oscillations to increasing doses of a drug and obtain single cell dose response curves. Because this method is very sensitive and measures single cell responses it has potential in drug discovery and characterization. Microtubules form a dynamic cytoskeleton, which is responsible for cell shape, intracellular transport and has an integral role in mitosis. A hallmark of microtubule organization is lateral interactions. Microtubules are bundles by proteins into dense structures. To estimate the contribution of this bundling process, we created a fractal model of microtubule organization. This model demonstrates that morphology of complex microtubule arrays can be explained by bundling alone. In summary we showed that advances in software for visualization, data analysis and modeling lead to new discoveries.
TNF wird zunächst als TypII-Transmembranprotein (mTNF) gebildet und erst anschließend durch spezifische Spaltung durch die Metalloprotease TACE zum löslichen Zytokin sTNF prozessiert. Da mTNF der alleinige Hauptaktivator des TNFR2 ist und sich bisherige Untersuchungen zum TNF-Signaling weitgehend auf sTNF konzentrierten, ist vergleichsweise wenig über TNFR2-vermittelte Signaltransduktion bekannt. An TNFR1 sind dagegen beide TNF-Varianten bioaktiv. Trotz intensiver Untersuchung des TNFR1-Signaling sind jedoch auch hier viele Fragen noch unbeantwortet. Derzeit existieren deshalb zum TNFR1-Signaling zwei verschiedene Modellvorstellungen nebeneinander. Im ersten Modell, dem Modell der Kompartmentalisation, bindet TRADD erst nach Rezeptorinternalisierung an TNFR1, genauso wie FADD und Caspase-8. Die Rezeptorinternalisierung nach Ligandenbindung gilt hier daher als Voraussetzung für die TRADD-Rekrutierung und für die Apoptoseinduktion. Im zweiten Modell, dem Modell zweier sequentiell arbeitender Signalkomplexe, bindet TRADD dagegen bereits im membrangebundenen Signalkomplex an TNFR1. Anschließend dissoziiert TRADD vom Rezeptor, um im Zytoplasma einen zweiten, apoptoseinduzierenden Komplex mit FADD und Caspase-8 zu formen. Um mehr über TNFR2 zu erfahren und um das TNFR1-Signaling besser zu verstehen, wurden in dieser Arbeit die Signaltransduktion und die Geninduktion über TNFR1 und TNFR2 nach Stimulation mit mTNF untersucht. Ziel war es letztlich, eine Methode zu etablieren, die es erlaubt, membrangebundene TNFR1- und TNFR2-Signalkomplexe getrennt zu isolieren. Dazu wurden zunächst nicht zu sTNF spaltbare TNFR1- bzw. TNFR2-spezifische mTNF-Varianten mit GST-Tag hinsichtlich Rezeptorbindung und Rezeptoraktivierung näher charakterisiert. Die selektive Bindung dieser mTNF-Varianten an TNFR1 bzw. TNFR2 konnte gezeigt werden. Auch der Nachweis ihre Funktionalität in Versuchen zur IL8-Induktion war möglich. Mit Hilfe der TNFR1-spezifischen mTNF-Variante gelang im GST-Fishing die Koimmunopräzipitation von TNFR1, TRADD und TRAF2 und damit die Isolierung des membrangebundenen Signalkomplexes des TNFR1. Mit Hilfe einer TNFR2-spezifischen Variante konnten dagegen TNFR2 und TRAF2 koimmunopräzipitiert werden, TRADD dagegen nicht. Somit ließen sich mit den rezeptorspezifischen Varianten von mTNF die Rezeptorsignalkomplexe des TNFR1 und TNFR2 getrennt isolieren. Interessant war dabei insbesondere die TRADD-Rekrutierung an TNFR1 im membrangebundenen TNFR1-Signalkomplex. Da die Internalisierung von TNFR1 nach mTNF-Stimulation schwer vorstellbar ist, bindet TRADD offensichtlich an TNFR1, ohne dass eine Rezeptorinternalisierung Voraussetzung wäre. Damit erscheint das Modell der Kompartmentalisation zumindest für mTNF wenig plausibel. Dagegen sind die bisher für mTNF erhobenen Daten mit einer TRADD-Dissoziation vom Rezeptor vereinbar, weshalb ein Modell zweier sequentiell arbeitender Signalkomplexe durchaus auch für mTNF Gültigkeit besitzen könnte.
- 77/83 allerdings inaktiv in Kulturen und Embryonen von Medaka. Dieser Unterschied wird durch Daten aus humanen ES-Zellkulturen unterstützt. Letztere sind ebenfalls komplett STAT3 unabhängig. Die BMP-Smad Kaskade wiederum ist in Medaka-Stammzellen aktiv, Antidifferenzierungsgene wie id2, die durch BMP direkt kontrolliert werden, sind dementsprechend exprimiert. Diese Daten stimmen wiederum mit dem Maussystem überein, während humane ES-Zellen diesbezüglich bislang nicht untersucht wurden. Die Interaktion zwischen verschiedenen Signalwegen ist ein bisher noch nicht gut verstandenes Gebiet. Die Integration verschiedener Signale ist aber speziell für Stammzellen, die ihr Differenzierungsschicksal von winzigen Abweichungen in der Signalmixtur abhängig machen, von entscheidender Bedeutung. Im zweiten Teil der hier vorgelegten Arbeit konnte eine Interaktion zwischen dem BMP-Rezeptor 1a und STAT3 nachgewiesen werden. Diese Interaktion ist offenbar Teil eines variablen Komplexes. Zum ersten Mal war es auch möglich, funktionale Konsequenzen für STAT3 nach Stimulierung des BMP-Rezeptors 1a zu dokumentieren. Nach Belegung des BMP-Rezeptors 1a mit dem mutierten BMP2-A34D wird STAT3 trotz Aktivierung durch Phosphorylierung an Tyrosin 705 im Zytoplasma von Maus Stammzellen festgehalten. Zusammengenommen konnte hier gezeigt werden, dass eine Interaktion zwischen den bislang als isoliert betrachteten Signalwegen BMP-Smad und STAT3 besteht. Des Weiteren wurde das Medaka-Stammzellkultursystem benutzt, um zu zeigen, dass STAT3 für die Pluripotenz von Stammzellen nur im Maussystem eine Rolle spielt, wohingegen BMPZielgene wie id2 in bislang allen getesteten ES-Zellkultursystemen aktiv sind.
Es wurde das Lebendnierenspendeprogramm der Universitätsklinik Würzburg anhand der statistischen Auswertung von 40 Transplantationen im Zeitraum von 1992-2003 untersucht. Dabei werden prä-, intra- und postoperative Verläufe von Spendern und Empfängern beschrieben sowie mit Ergebnissen anderer Transplantationszentren verglichen. Die Ergebnisse sind hierbei als zufriedenstellend zu bezeichnen.
Zur Abklärung malignitätsverdächtiger Befunde in der Brust benötigt man eine effektive minimal-invasive Methode. Diese retrospektive Studie evaluiert die Wertigkeit der Vakuumstanzbiopsie mit manueller Führung. Es zeigt sich, dass hier eine unkomplizierte und ausreichende Gewinnung von Gewebe und somit eine zuverlässige Einschätzung von mammographischen Auffälligkeiten möglich ist.
Introduction: This study investigates the role of Wolbachia bacteria in the pathogenesis of O. volvulus keratitis in a mouse model. Wolbachia bacteria are essential symbionts of most filarial nematodes of importance for mankind. Methods: Using a mouse model for river blindness in which soluble extracts of filarial nematodes are injected in the corneal stroma, changes in stromal thickness and haze of the cornea are observed by in vivo confocal microscopy, followed by immunohistochemical staining for neutrophils and PECAM-1, as well as ELISA of corneal chemokines. Reactions to filarial extracts containing Wolbachia are compared to those without the endosymbiont. Results: The approach of characterizing Wolbachia’s role in river blindness in this study is threefold. Firstly, Wolbachia-depleted extracts from doxycycline treated onchocerciasis patients led to a diminished inflammatory response in corneas of C57BL/6 mice compared to untreated, i.e. Wolbachia containing antigen. The decreased cell recruitment observed with doxycycline treated extracts involved neutrophils, but not eosinophils. This finding demonstrated that the presence of Wolbachia increases neutrophil recruitment. Secondly, extracts from Wolbachia-containing B. malayi revealed markedly more pathology than endosymbiont-free A. viteae antigen. This again pointed at the role of Wolbachia in development of disease. Thirdly, Toll-like Receptor 4 (TLR4) dependence was shown to exist for the inflammatory response to Wolbachia harboring O. volvulus antigen by looking at the corneal pathology in TLR4-mutant C3H/HeJ mice, compared to the wild-type C3H/HeN strain. Investigating further Wolbachia mediated mechanisms of neutrophil recruitment to the cornea, this study also showed that expression of the adhesion molecule PECAM-1 in limbal vessels, as well as upregulation of the CXC chemokines KC and MIP-2 were dependent on the presence of functional TLR4 and Wolbachia respectively. Conclusions: This study indicates that the innate immune system and Wolbachia endobacteria play an important role in the inflammatory response associated with the pathogenesis of onchocerca keratitis, suggesting a complete alteration in our understanding of the immunopathology of filariasis.
Ein Tumor stellt nicht nur eine Ansammlung entarteter Zellen dar, sondern ist vielmehr ein komplexes Pseudoorgan, das aus Tumorzellen und aus mit ihnen assoziierten „normalen“ Zelltypen, wie Fibroblasten, Endothelzellen und Makrophagen, den sogenannten Tumorstromazellen, besteht. Die Tumorstromazellen wurden von den Tumorzellen dahingehend konditioniert, dass sie das Tumorwachstum und -progression fördern. Im Rahmen der vorliegenden Arbeit wurde die Bedeutung von zwei Oberflächenmolekülen, nämlich CD147 und CD28, für solche im Tumorstroma stattfindenden Interaktionen im syngenen murinen B16 Melanommodell untersucht. Rolle von CD147 für MMP Expression, Neoangiogenese und Metastasierung CD147, das von Tumorzellen exprimiert wird, wird als ein Faktor angesehen, der auf benachbarten Stromazellen die Expression von MMPs induziert. MMPs sind essentiell für den Umbau der extrazellulären Matrix und der Basalmembranen und somit für die Invasion und Metastasierung des Tumors essentiell. Daneben gibt es erste Hinweise, dass CD147 auch die Induktion von vascular endothelial growth factor (VEGF) vermittelt und damit die Tumorangiogenese fördert. In dem eingesetzten Melanommodell war überraschenderweise kein Unterschied hinsichtlich der Expression von MMP-2, MMP-9 und MT1-MMP in Abhängigkeit von der CD147 Expression nachweisbar. Die in vitro Kokultur der Melanomzellen mit unterschiedlichen murinen Fibroblasten zeigte zudem, dass weder CD147+ noch CD147- Melanomzellen die Expression von MMP-2 oder MMP-9 in den Fibroblasten veränderten. Als eindeutige Effekte des CD147 knock downs wurde aber eine reduzierte VEGF Expression in vivo einhergend mit einer gehemmten Tumorangiogenese, sowie einer reduzierten Metastasierung festgestellt. Es konnte somit die Funktion von CD147 in dem gewählten Modell als angiogenetischer, jedoch als MMP unabhängiger, Metastasierungsfaktor demonstriert werden. Einfluss von CD28 auf antitumorale Immunantworten CD28 ist ein kostimulatorisches Molekül, das zusammen mit dem TCR für eine effiziente Stimulation von T-Lymphozyten wesentlich ist. In CD28 k.o Mäusen fand sich im Vergleich zu Wildtyp Kontrolltieren eine verminderte Effektivität von prophylaktischen anti-Tumor Impfungen, die sich in einem beschleunigten Tumorwachstum sowie einer erhöhten Tumorlast auswirkten. Die Frequenz von Vakzine induzierten TRP-2180-188 /Kb reaktiven CD8+ T-Zellen in TIL von Tumoren war aber in beiden Genotypen gleich. Dagegen war die Anzahl IFN- produzierender TRP-2180-188 /Kb reaktiver T-Zellen sowie die Fähigkeit der TRP-2180-188 /Kb reaktiven T-Zellen zu lysieren, in den CD28-defizienten Mäusen deutlich geringer. Diese Beobachtungen legen nahe, dass CD28-vermittelte kostimulatorische Signale im gewählten Modell weniger für die initiale Expansion als für die Differenzierung funktioneller tumorspezifischer CD8+ T-Effektorzellen eine wesentliche Funktion einzunehmen scheinen.
In radiation accidents biological methods are used in dosimetry, if the radiation dose could not be measured by physical methods. The knowledge of individual dose is a prerequisite for planning a medical treatment and for health risk evaluations. In the present work two biodosimetrical assays were calibrated in young patients who were treated with radioiodine for thyroid cancer. Patients were from Belarus. They suffered from radiation induced thyroid cancer as a consequence of the Chernobyl reactor accident. In radioiodine therapy (RIT) bone marrow and lymphatic organs are exposed to ionizing radiation at doses of 0.1 to 0.75 Sv within about 2 days. Since several RIT have to be applied with interval between each of them from 6 months up to approximately 1 year, total dose can be up to 2 Sv within 2 to 3 years. The dose for thyroid tissue is approximately 1000 times higher. The dose-response relationship was measured by the T-cell receptor test (TCR test) in T4 lymphocytes with and without in vitro incubation or by the micronucleus assay in transferrin receptor positive reticulocytes (MN-Tf-Ret test). In all these assays, the frequency of radiation-induced mutants of blood cells is measured using flow cytometry. The TCR test is a cumulative biodosimeter, which measures the total radiation dose within the last 5 to 10 years, whereas the result of the MN-Tf-Ret test reflects the radiation dose of approximately 24 hours interval. It takes 8 hours and 3 days to perform TCR and MN-Tf-Ret tests respectively. Calibration curves based on radioiodine treated patients can be used for dose estimation in humans, if the radiation conditions correspond to those in RIT. This limits their applicability to low dose-rate β- and γ-irradiation and to doses per session not higher than about 0.5 Sv. If higher doses or dose-rates as well as the other types of ionizing radiation are involved, calibration curves in animals are indispensable. In the case MN-Tf-Ret test mouse models are established and may be used. The TCR assay was performed in 72 thyroid cancer patients aged between 14 and 25. T-cell mutant frequency (Mf) reaches its maximum only after half a year following the RIT. Then it declines exponentially. This decline could be described by the 3 parameter single exponential decay function. Based on this equation, the radiation dose could be calculated when the Mf and the time interval since exposure are known. Furthermore, the experimentally measured Mf value, which significantly exceeds the corresponding calculated Mf value would indicate an individual with higher radiosensitivity. However, among our patients there were none. The reticulocytes micronuclei test (MN-Tf-Ret) was performed in 46 radioiodine treated patients. When measuring the MN frequency (f(MN-Tf-Ret)) the measured cell fraction should be limited only to the youngest cohort of reticulocytes, because all the micronucleated erythrocytes are quickly removed from the peripheral blood by spleen. Thus, the MN test was performed only in CD71 positive (having transferring receptor) reticulocytes. These reticulocytes just entered the peripheral blood flow from red marrow. The MN frequency was measured before the therapy and then every day after the irradiation until day 7. MN frequency curve has typical shape with latent period for days 0 to 3. Then there is a sharp increase in MN frequency which lasts for 24 hours and could start between days 3 and 4. In the following days the MN frequency is dropping to its base level that equals the one before the treatment. The decay of MN frequency is depending on the half-life of radioiodine in the patient organism. If the half-life is low, then the increased f(MN-Tf-Ret) lasts shorter and vice versa. It was shown that the MN frequency curve could be described by the model where all the micronuclei arise only through the last mitosis of erythroblasts in the red marrow and the MN frequency is proportional to the radiation dose in the last cell cycle. The shape of this curve depends on the cell kinetics of erythropoiesis on one side and the exponential decay of radioiodine activity on the other. To the best of our knowledge, the MN-Tf-Ret test was applied in the present study for the first time in biological dosimetry.
The RS1 protein, a 67 kDa protein, encoded by an intronless single copy gene that was only detected in mammals, mediates transcriptional and post-transcriptional down-regulation of the sodium-D-glucose co-transporter SGLT1. The short-term post-transcriptional down-regulation of SGTL1 by RS1 has been shown to occur at the trans-Golgi network (TGN). In the present study, two tripeptides from the human RS1 protein (hRS1), GlnCysPro and GlnSerPro, that induce the post-transcriptional down-regulation of SGLT1 at the TGN, were identified. The application of the tripeptides led to 40-50% reduction of the amount of the SGLT1 protein in the plasma membrane, which correlated to the degree of decrease in SGLT1-mediated glucose transport. For the short-term down-regulation of SGLT1 by the tripeptides, the effective intracellular concentrations IC50 values of 2.0 nM (GlnCysPro, QCP) and 0.16 nM (GlnSerPro, QSP) were estimated. The observed down-regulation of SGLT1 by the tripeptides QCP and QSP, similar to hRS1 protein, was attenuated by different intracellular monosaccharides including nonmetabolized methyl-α-D-glucopyranoside and 2-deoxyglucose. On the contrary, the short-term inhibition of the hOCT2 by QCP could only be observed after rising of intracellular concentration of AMG. QCP and QSP are transported by H+-peptide cotransporter PEPT1 that is co-located with SGLT1 in the small intestinal enterocytes and thereafter effectively down-regulate hSGLT1-mediated transport of AMG. The data indicates that orally applied tripeptides QCP or QSP can be used to down-regulate D-glucose absorption in small intestine and used for treatment of obesity and diabetes mellitus.
Bei morbider Adipositas (BMI > 40 kg/m²), von der in Deutschland 1,6% der übergewichtigen Frauen und 0,5% der übergewichtigen Männer (BMI > 25 kg/m²) betroffen sind, erweisen sich für eine ausreichende und dauerhafte Gewichtsreduktion nur chirurgische Maßnahmen, wie z.B. laparoskopisches Gastric banding als effektiv. Allerdings fehlen Studien zur langfristigen Effektivität. In der vorliegenden Katamnesestudie zur Langzeiteffektivität von Gastric banding acht Jahre nach stattgefundener Operation wurden Veränderungen in Gewicht, depressiver Symptomatik, Selbstakzeptanz, Essverhalten und aufgetretene Komplikationen untersucht.
Hintergrund: Ziel dieser Dissertation ist es, die Faktorenstruktur von Items bereits existierender englischsprachiger biographischer Fragebögen genauer zu betrachten und valide Variablen zur Untersuchung eines oder mehrerer Persönlichkeitsmerkmale zusammenzustellen. Methode: Zunächst wird ein Rohfragebogen mit insgesamt 228 Items, welche aus der englischsprachigen Literatur übersetzt wurden, gebildet und an die erwachsene Bevölkerung mittels Gelegenheitsauswahl verteilt. Die Items des Primärfragebogens beziehen sich auf alltägliche Situationen, Verhalten, Gefühle und Gedanken, die im Kindes- und Jugendalter erlebt wurden. Es wird ein geschlossenes Fragebogendesign angewendet, so dass der Teilnehmer zwischen den Antwortmöglichkeiten „stimmt“ und „stimmt nicht“ auswählt. Die statistische Auswertung erfolgt mit Hilfe der Faktorenanalyse. Ergebnis: Bei einer Rücklaufquote von 68,2 Prozent ergibt sich eine Stichprobengröße von 445 Teilnehmern. Die Altersspanne beläuft sich zwischen 15 und 77 Jahren, bei einem Median von 34,98. 58,3 Prozent (259) der Teilnehmer sind weiblich, 41,7 Prozent (185) sind männlich. Nach Auswertung der Daten der Faktorenanalyse und ausführlicher statistischer und inhaltlicher Kontrolle ergibt die 1-Faktorenlösung eine valide und homogene Kurzskala bestehend aus 15 Items, der inhaltlich die Überschrift Selbstbewusstsein zugeordnet werden kann. Zusammenfassung: Das individuelle Selbstbewusstsein spielt in der Persönlichkeit eines Menschen eine besondere Rolle. Erlebnisse und Lebenssituationen in der Kindheit und Jugend sind maßgebend für die Entfaltung des Selbstbewusstseins eines Einzelnen. Vor allem ein geringer Ausprägungsgrad stellt eine erhebliche Beeinträchtigung in der Entwicklung eines Menschen dar und kann für psychologische Erkrankungen im Erwachsenenalter verantwortlich sein, die ein therapeutisches Eingreifen notwendig machen. Schlussfolgerung: Mit Hilfe einer biographischen Kurzskala kann der behandelnde Arzt das Selbstbewusstsein seines Patienten anhand von Prozenträngen quantifizieren. Es ist vorstellbar, diese für die Gesamtbevölkerung konzipierte Skala zukünftig als Teil eines umfangreichen Messinventars in der ärztlichen Anamnese einzusetzen.
Brassicaceae and a few related plant families are characterized by possession of the glucosinolate-myrosinase system. Glucosinolates are amino-acid derived allelochemicals which are hydrolysed upon tissue damage by myrosinase enzymes to produce various degradation products which can be toxic for generalist insects. The larvae of the crucifer-specialist Athalia rosae, the turnip sawfly, sequester glucosinolates into their haemolymph. The role of the glucosinolate-myrosinase system for the interaction of the turnip sawfly with Brassicaceae was examined in this study from two different perspectives: variation within individual plants and between plant species. The plant responses to the feeding by herbivores and the short-term effects this induction had on insect behaviour were investigated in white mustard. Furthermore, plants can use multiple defences. Hence correlations of glucosinolates and myrosinase activities with other defences and nutritional quality and their long-term effects on the development of the insects were investigated in seven different plant species.
This thesis is devoted to the study of computational complexity theory, a branch of theoretical computer science. Computational complexity theory investigates the inherent difficulty in designing efficient algorithms for computational problems. By doing so, it analyses the scalability of computational problems and algorithms and places practical limits on what computers can actually accomplish. Computational problems are categorised into complexity classes. Among the most important complexity classes are the class NP and the subclass of NP-complete problems, which comprises many important optimisation problems in the field of operations research. Moreover, with the P-NP-problem, the class NP represents the most important unsolved question in computer science. The first part of this thesis is devoted to the study of NP-complete-, and more generally, NP-hard problems. It aims at improving our understanding of this important complexity class by systematically studying how altering NP-hard sets affects their NP-hardness. This research is related to longstanding open questions concerning the complexity of unions of disjoint NP-complete sets, and the existence of sparse NP-hard sets. The second part of the thesis is also dedicated to complexity classes but takes a different perspective: In a sense, after investigating the interior of complexity classes in the first part, the focus shifts to the description of complexity classes and thereby to the exterior in the second part. It deals with the description of complexity classes through leaf languages, a uniform framework which allows us to characterise a great variety of important complexity classes. The known concepts are complemented by a new leaf-language model. To a certain extent, this new approach combines the advantages of the known models. The presented results give evidence that the connection between the theory of formal languages and computational complexity theory might be closer than formerly known.
