Objectives
Handball is associated with a high risk of overuse shoulder injury. This study investigated if an injury prevention programme effectively reduces overuse injury to the throwing shoulder of handball athletes.
Methods
61 men’s and women’s handball teams (u-19 and senior athletes) were cluster-randomised into an intervention and a control group in the 2019–2020 season. Players of the intervention group regularly carried out an injury prevention programme. Both groups documented overuse shoulder injuries via an online questionnaire every second week. The primary endpoint was the prevalence of overuse injury to the throwing shoulder. Secondary endpoints were the influence of compliance on the primary endpoint and intensity of overuse shoulder symptoms measured by a shortened, handball-specific Western Ontario Shoulder Index (WOSI).
Results
31 teams (295 players) in the intervention group and 30 teams (284 players) in the control group were included for analyses. The overall questionnaire response rate was 61%. The average prevalence of overuse shoulder injury did not significantly differ between the intervention group (n=109, 38.4% (95% CI 32.9% to 44.2%)) and the control group (n=106, 35.9% (95% CI 30.7% to 41.6%), p=0.542). Compliance with the intervention programme did not significantly affect overuse shoulder injury (p=0.893). Using generalised estimating equations for WOSI, the estimated mean for the intervention group was 44.6 points (95% CI 42.0 to 47.1) and 47.6 points for the control group (95% CI 44.9 to 50.3, p=0.111).
Conclusions
A multicomponent exercise programme using rubber bands and stretching did not significantly reduce the prevalence or symptoms of overuse throwing shoulder injury in handball athletes of both sexes. Randomised controlled study; level of evidence I.
Background
To identify injury patterns and mechanisms in professional men’s basketball by means of video match analysis.
Methods
In Germany, injuries are registered with the statutory accident insurance for professional athletes (VBG) by clubs or club physicians as part of occupational accident reporting. Moderate and severe injuries (absence of > 7 days) sustained during basketball competition in one of four seasons (2014–2017 and 2018–2019) in the first or second national men’s league in Germany were prospectively analyzed using a newly developed standardized observation form. Season 2017–2018 was excluded because of missing video material.
Results
Video analysis included 175 (53%) of 329 moderate and severe match injuries. Contact patterns categorized according to the different body sites yielded eight groups of typical injury patterns: one each for the head, shoulders, and ankles, two for the thighs, and three for the knees. Injuries to the head (92%), ankles (76%), shoulders (70%), knees (47%), and thighs (32%) were mainly caused by direct contact. The injury proportion of foul play was 19%. Most injuries (61%) occurred in the central zone below the basket. More injuries occurred during the second (OR 1.8, p = 0.018) and fourth quarter (OR 1.8, p = 0.022) than during the first and third quarter of the match.
Conclusion
The eight identified injury patterns differed substantially in their mechanisms. Moderate and severe match injuries to the head, shoulders, knees, and ankles were mainly caused by collision with opponents and teammates. Thus, stricter rule enforcement is unlikely to facilitate safer match play.
To date, no consensus exists regarding the best surgical management of isolated, micro-traumatic long thoracic nerve (LTN) paresis. Our hypothesis was that a combined decompression of the LTN at two potential locations for entrapment would be effective in the management of dynamic LTN paresis. We report on twelve patients with isolated LTN parersis, with tenderness at two entrapment sites, who underwent bifocal LTN decompression after undergoing unsuccessful conservative treatment for at least 6 months; all patients had preoperative electrodiagnostic studies that confirmed the paresis and ruled out peripheral neuritis. Clinical and electrical improvements were observed in eight patients (67%) regarding shoulder flexion, shoulder abduction, and Quick-DASH scores. Four patients (33%) did not improve after surgery. The results corroborate our hypothesis that a bifocal LTN decompression can be an effective and reliable therapeutic option in more than half of a very selective patient population suffering from serratus anterior muscle deficiency.
This work developed during the first funding period of the subproject B05 in the framework of the interdisciplinary research consortium TRR 225 ‘From the Fundamentals of Biofabrication toward functional Tissue Models’ and was part of a cooperation between the Orthopedic Department represented by Prof. Dr. Regina Ebert and the Institute of Organic Chemistry represented by Prof. Dr. Jürgen Seibel.
This project dealed with cellular behavior during the bioprinting process and how to influence it by modifying the cell glycocalyx with functional target molecules. The focus was on the impact of potential shear stress, that cells experience when they get processed in thermoresponsive bioinks, and a way to increase the cell stiffness via metabolic glycoengineering to attenuate shear forces. For the characterization of the metabolic glycoengineering, four different peracetylated and four non-acetylated modified monosaccharides (two mannose and two sialic acid sugars) were tested in primary human mesenchymal stromal cells (hMSC) and telomerase-immortalized hMSC (hMSC-TERT). Viability results demonstrated a dose-dependent correlation for all sugars, at which hMSC-TERT seemed to be more susceptible leading to lower viability rates. The assessment of the incorporation efficiencies was performed by click chemistry using fluorescent dyes and revealed also a dose-dependent correlation for all mannose and sialic acid sugars, while glucose and galactose variants were not detected in the glycocalyx. However, incorporation efficiencies were highest when using mannose sugars in the primary hMSC. A subsequent analysis of the temporal retention of the incorporated monosaccharides showed a constant declining fluorescence signal up to 6 d for azido mannose in hMSC-TERT, whereas no signal could be detected for alkyne mannose after 2 d. Investigation of the differentiation potential and expression of different target genes revealed no impairment after incubation with mannose sugars, indicating a normal phenotype for hMSC-TERT. Following the successful establishment of the method, either a coumarin derivative or an artificial galectin 1 ligand were incorporated into the cell glycocalyx of hMSC-TERT as functional target molecule. The biophysical analysis via shear flow deformation cytometry revealed a slightly increased cell stiffness and lowered fluidity for both molecules. A further part of this project aimed to control lectin-mediated cell adhesion by artificial galectin 1 ligands. As that hypothesis was settled in the work group of Prof. Dr. Jürgen Seibel, this work supported with an initial characterization of galectin 1 as part of the hMSC biology. A stable galectin 1 expression at gene and protein level in both hMSC and hMSC-TERT could be confirmed, at which immunocytochemical stainings could detect the protein only in the glycocalyx. The treatment of hMSC-TERT with a galectin 1 ligand in different concentrations did not show an altered gene expression of galectin 1. However, these first data in addition to the investigation of stiffness confirmed the applicability of specific and artificial

galectin 1 ligands in biofabrication approaches to alter cell properties of hMSC. To conclude, metabolic glycoengineering has been successfully implemented in hMSC and hMSC-TERT to introduce glycocalyx modifications which reside there for several days. A proof of concept was carried out by the increase of cell stiffness and fluidity by the incorporation of a coumarin derivative or an artificial galectin 1 ligand.
For the characterization of shear stress impact on cells after printing in thermoresponsive bioinks, the processing of hMSC-TERT (mixing or additionally printing) with Pluronic F127 or Polyoxazoline-Polyoxazine (POx-POzi) polymer solution was investigated. While there were no changes in viability when using POx-POzi bioink, processing with Pluronic F127 indicated slightly lower viability and increased apoptosis activity. Assessment of cellular responses to potential shear stress showed no reorganization of the cytoskeleton independent of the bioink, but highly increased expression of the mechanoresponsive proto-oncogene c Fos which was more pronounced when using Pluronic F127 and just mixed with the bioinks. Interestingly, processing of the mechanoresponsive reporter cell line hMSC-TERT-AP1 revealed slightly elevated mechanotransduction activity when using POx-POzi polymer and just mixed with the bioinks as well. In conclusion, hMSC-TERT embedded in thermoresponsive bioinks might shortly experience shear stress during the printing process, but that did not lead to remarkable cell damage likely due to the rheological properties of the bioinks. Furthermore, the printing experiments also suggested that cells do not sense more shear stress when additionally printed.
Metabolic glycoengineering enables a directed modification of cell surfaces by introducing target molecules to surface proteins displaying new features. Biochemical pathways involving glycans differ in dependence on the cell type; therefore, this technique should be tailored for the best results. We characterized metabolic glycoengineering in telomerase-immortalized human mesenchymal stromal cells (hMSC-TERT) as a model for primary hMSC, to investigate its applicability in TERT-modified cell lines. The metabolic incorporation of N-azidoacetylmannosamine (Ac\(_4\)ManNAz) and N-alkyneacetylmannosamine (Ac\(_4\)ManNAl) into the glycocalyx as a first step in the glycoengineering process revealed no adverse effects on cell viability or gene expression, and the in vitro multipotency (osteogenic and adipogenic differentiation potential) was maintained under these adapted culture conditions. In the second step, glycoengineered cells were modified with fluorescent dyes using Cu-mediated click chemistry. In these analyses, the two mannose derivatives showed superior incorporation efficiencies compared to glucose and galactose isomers. In time-dependent experiments, the incorporation of Ac\(_4\)ManNAz was detectable for up to six days while Ac\(_4\)ManNAl-derived metabolites were absent after two days. Taken together, these findings demonstrate the successful metabolic glycoengineering of immortalized hMSC resulting in transient cell surface modifications, and thus present a useful model to address different scientific questions regarding glycosylation processes in skeletal precursors.
Im Rahmen der vorliegenden Studie wurde an 220 Patienten, die zwischen 1988 und 2007 im König-Ludwig-Haus in Würzburg durch einen Operateur wegen rezidivierender, überwiegend posttraumatischer ventraler Schulterinstabilität offen oder arthroskopisch mittels (modifizierter) Bankart-Prozedur operiert wurden, der „Instability Severity Index Score (ISIS)“ so erhoben, wie er aus den präoperativen Unterlagen zu ermitteln war. Alle Patienten wurden nach postoperativen Rezidivluxationen befragt und die Schulterfunktion wurde mittels standardisiertem und validiertem Fragebogen durch den „Constant Score“ und den „Oxford Shoulder Instability Score“ untersucht.
Ziel der Studie war es, den von Balg und Boileau 2007 vorgestellten „Instability Severity Index Score“ (ISIS) auf seine Aussagekraft hin am vorliegenden Kollektiv zu überprüfen. Zeitgleich sollten ein Vergleich der offenen mit den arthroskopischen Stabilisierungen sowie eine Analyse der Ursachen der Rezidivluxationen erfolgen.
Insgesamt kam es in acht Fällen zu Rezidivluxationen (3,6 %). Die offen Operierten wiesen eine Rate von 3,1 %, die Gruppe der arthroskopisch Operierten 8,7 % Rezidive auf. Patienten mit weniger oder gleich sechs Punkten im ISIS hatten in 2,7 % Reluxationen, Patienten mit mehr als sechs Punkten in 8,1 %.
Patienten, die rückblickend gemäß der Empfehlung aus dem ISIS operiert wurden, hatten in 5,3 % Rezidivluxationen. Patienten, die entgegen der Empfehlung operiert wurden, in 3,5 %. Alle Unterschiede waren statistisch nicht signifikant. In allen Gruppen konnten in den funktionellen Scores sehr gute Ergebnisse mit durchschnittlich über 87 % im alters- und geschlechtsadaptierten Constant Score und über 42 Punkten im Oxford Shoulder Instability Score ohne signifikante Unterschiede erzielt werden. Von den insgesamt acht Patienten mit Reluxationen lagen von zwei Patienten CT-Untersuchungen nach aufgetretener Reluxation vor. In beiden Fällen konnten signifikante Glenoidranddefekte gefunden werden.
Aus Sicht der erhobenen Daten und der erzielten Ergebnisse ist der ISIS als nützlich zur präoperativen Risikobewertung sowie zur Entscheidung über das operative Vorgehen einzuschätzen, wobei er keine imperative Handlungsanweisung darstellen sollte. Die Empfehlung zum Korakoidtransfer nach Latarjet ab sieben Punkten im ISIS kann anhand dieser Daten nicht bestätigt werden. Vielmehr konnte gezeigt werden, dass eine offene Bankart-Operation mit selektivem Kapselshift sehr gute Langzeitergebnisse bezüglich der Reluxationsraten und der funktionellen Ergebnisse liefert. Im Hinblick auf die erzielten Ergebnisse und Fehleranalysen ist weiterhin festzuhalten, dass bei Verdacht auf einen Glenoiddefekt in der Regel eine CT mit 3D-Rekonstruktion und Seiten-vergleich erfolgen sollte, um die Indikation zum offenen Knochenblocktransfer nicht zu verpassen. Offene und arthroskopische Stabilisierungen können bei richtiger Indikationsstellung kurz- und mittelfristig vergleichbar gute Ergebnisse liefern. Langfristig aber scheint das minimal-invasive Vorgehen höhere Raten an Rezidivluxationen aufzuweisen. Wie auch in dieser Arbeit gezeigt werden konnte, ist ein langer Beobachtungszeitraum bei Studien, die das klinische Ergebnis von Schulterstabilisierungen untersuchen, sehr wichtig, um das wahre Ausmaß an postoperativen Rezidivinstabilitäten zu erfassen.
Background: Mesenchymal stem cells (MSCs) and their chondrogenic differentiation have been extensively investigated in vitro as MSCs provide an attractive source besides chondrocytes for cartilage repair therapies. Here we established prototype foamyviral vectors (FVV) that are derived from apathogenic parent viruses and are characterized by a broad host range and a favorable integration pattern into the cellular genome. As the inflammatory cytokine interleukin 1 beta (IL1β) is frequently present in diseased joints, the protective effects of FVV expressing the human interleukin 1 receptor antagonist protein (IL1RA) were studied in an established in vitro model (aggregate culture system) of chondrogenesis in the presence of IL1β.
Materials and Methods: We generated different recombinant FVVs encoding enhanced green fluorescent protein (EGFP) or IL1RA and examined their transduction efficiencies and transgene expression profiles using different cell lines and human primary MSCs derived from bone marrow-aspirates. Transgene expression was evaluated by fluorescence microscopy (EGFP), flow cytometry (EGFP), and ELISA (IL1RA). For evaluation of the functionality of the IL1RA transgene to block the inhibitory effects of IL1β on chondrogenesis of primary MSCs and an immortalized MSC cell line (TERT4 cells), the cells were maintained following transduction as aggregate cultures in standard chondrogenic media in the presence or absence of IL1β. After 3 weeks of culture, pellets were harvested and analyzed by histology and immunohistochemistry for chondrogenic phenotypes.
Results: The different FVV efficiently transduced cell lines as well as primary MSCs, thereby reaching high transgene expression levels in 6-well plates with levels of around 100 ng/ml IL1RA. MSC aggregate cultures which were maintained in chondrogenic media without IL1β supplementation revealed a chondrogenic phenotype by means of strong positive staining for collagen type II and matrix proteoglycan (Alcian blue). Addition of IL1β was inhibitory to chondrogenesis in untreated control pellets. In contrast, foamyviral mediated IL1RA expression rescued the chondrogenesis in pellets cultured in the presence of IL1β. Transduced MSC pellets reached thereby very high IL1RA transgene expression levels with a peak of 1087 ng/ml after day 7, followed by a decrease to 194 ng/ml after day 21, while IL1RA concentrations of controls were permanently below 200 pg/ml.
Conclusion: Our results indicate that FVV are capable of efficient gene transfer to MSCs, while reaching IL1RA transgene expression levels, that were able to efficiently block the impacts of IL1β in vitro. FVV merit further investigation as a means to study the potential as a gene transfer tool for MSC based therapies for cartilage repair.
Vergleichende Genexpressions-Analyse unterschiedlicher Populationen mesenchymaler Stammzellen

Neben den omnipotenten embryonalen Stammzellen existieren im menschlichen Körper adulte mesenchymale Stammzellen (MSZ). Diese Zellen sind in mesenchymalen Geweben über den gesamten Organismus verteilt und sorgen für die Entwicklung und Erneuerung von mesenchymalen Geweben wie Knochen, Knorpel und Bändern. Daher gelten die MSZ im Gegensatz zu den omnipotenten embryonalen Stammzellen als multipotent. Diese verschiedenen MSZ stellen keine homogene Population dar, zeigen aber sowohl in vivo und auch in vitro ein ähnliches Differenzierungsverhalten. In der vorliegenden Arbeit wurde nun eine aus den Knochentrabekeln selbst isolierte MSZ-Population, so genannte bhMSZ, mit MSZ aus dem Knochenmark, mhMSZ genannt, mittels Array-Analyse miteinander verglichen. Die technische Evaluation des Array respektive der zugehörigen SAM-Analyse (significance analysis of microarrays) mittels konventioneller oder Real-Time PCR diente dazu, die Verlässlichkeit der Aussage der Hybridisierungsverfahren zu überprüfen. Dies wurde mit einem Set an ausgewählten Genen durchgeführt, die signifikant differentiell exprimiert waren, und die im Rahmen der Stammzellbiologie relevant erschienen. Die Analyse zeigte, dass die Übereinstimmung der Aussage im Array in über 80 % mit den Ergebnissen der RT-PCR kongruent war. Auf Grund starker interindividueller Schwankungen zeigte sich aber auch, dass die Anzahl der Spender 5 nicht unterschreiten sollte. Im Rahmen der Untersuchungen ergab sich, dass offenbar bei MSZ der Passage 0 eine Kontamination der MSZ mit Plasmazellen vorliegt. Weitere Versuche zeigten, dass erst das Passagieren der MSZ kontaminierende Plasmazellen weitgehend aus der Zellkultur entfernte. Aus diesem Grund wurde in einer weiteren Array Analyse das Transkriptom von MSZ aus Knochentrabekeln mit MSZ aus dem Knochenmark in Passage 1 verglichen. Es zeigten sich in einer stringenten SAM-Analyse keine Unterschiede im Transkriptom. Für klinische Anwendungen scheinen die bhMSZ daher auf Grund der aufwendigeren Isolierung und des dennoch eher geringen Zellgewinns nicht im gleichen Maß für klinische Anwendungen geeignet wie mhMSZ.
Background:
To prevent bone loss in hip arthroplasty, several short stem systems have been developed, including the Mayo conservative hip system. While there is a plethora of data confirming inherent advantages of these systems, only little is known about potential complications, especially when surgeons start to use these systems.
Methods:
In this study, we present a retrospective analysis of the patients’ outcome, complications and the complication management of the first 41 Mayo conservative hips performed in 37 patients. For this reason, functional scores, radiographic analyses, peri- and postoperative complications were assessed at an average follow-up of 35 months.
Results:
The overall HHS improved from 61.2 pre-operatively to 85.6 post-operatively. The German Extra Short Musculoskeletal Function Assessment Questionnaire (XSFMA-D) improved from 30.3 pre-operatively to 12.2 post-operatively. The most common complication was an intraoperative non-displaced fracture of the proximal femur observed in 5 cases (12.1%). Diabetes, higher BMI and older ages were shown to be risk factors for these intra-operative periprosthetic fractures (p < 0.01). Radiographic analysis revealed a good offset reconstruction in all cases.
Conclusion:
In our series, a high complication rate with 12.1% of non-displaced proximal femoral fractures was observed using the Mayo conservative hip. This may be attributed to the flat learning curve of the system or the inherent patient characteristics of the presented cohort."
Introduction
The goals of successful bi-compartmental knee arthroplasty are to achieve correct fit and positioning of the implant, while appropriately correcting the mechanical alignment of the leg after surgery. As these requirements are not always reliably fulfilled using off-the-shelf implant systems, newer approaches for bi-compartmental resurfacing have been explored.
Material and methods
In this article we report the radiographic results of 30 patients with anteromedial osteoarthritis (OA) who were treated with a novel patient-specific fixed-bearing bi-compartmental knee resurfacing system using custom-made implants and instruments. Utilizing standardized pre- and postoperative radiographic analyses (based on anterior-posterior and lateral, anterior-posterior weight-bearing full-length radiographs, patella skyline views and preoperative computed tomography (CT) scanning) implant fit and positioning as well as correction of the mechanical axis (hip-knee-ankle angle, HKA) were determined.
Results
On average, HKA was corrected from 173.4 ±3.47° preoperatively to 179.4 ±2.85° postoperatively. The coronal femoro-tibial angle was corrected on average 5.61°. The preoperative tibial slope measured on lateral views was 6.38 ±2.4°, while the average slope in the CT-based planning protocol (iView) was 6.14 ±2.40°. Postoperative lateral tibial slope was determined to be 5.77 ±1.97°. The thickness of the posterior femoral cuts was measured intraoperatively and, in all cases, corresponded well to the targeted thickness of the cuts provided by the iView. The joint line was preserved in all cases and the average Insall-Salvati index was 1.078 ±0.11 pre- and 1.072 ±0.11 postoperatively. The fit of the implant components measured by over- or underhang was excellent throughout (< 1.01 mm).
Conclusions
Custom-made bicompartmental knee arthroplasty can ensure optimized fitting and positioning of the implant with restoration of the leg axis. These implants could be considered as an alternative primary solution for knee surgeons treating bi-compartmental disease.
Background
For improved outcomes in total knee arthroplasty (TKA) correct implant fitting and positioning are crucial. In order to facilitate a best possible implant fitting and positioning patient-specific systems have been developed. However, whether or not these systems allow for better implant fitting and positioning has yet to be elucidated. For this reason, the aim was to analyse the novel patient-specific cruciate retaining knee replacement system iTotal (TM) CR G2 that utilizes custom-made implants and instruments for its ability to facilitate accurate implant fitting and positioning including correction of the hip-knee-ankle angle (HKA).
Methods
We assessed radiographic results of 106 patients who were treated with the second generation of a patient-specific cruciate retaining knee arthroplasty using iTotal\(^{TM}\) CR G2 (ConforMIS Inc.) for tricompartmental knee osteoarthritis (OA) using custom-made implants and instruments. The implant fit and positioning as well as the correction of the mechanical axis (hip-knee-ankle angle, HKA) and restoration of the joint line were determined using pre- and postoperative radiographic analyses.
Results
On average, HKA was corrected from 174.4 degrees +/- 4.6 degrees preoperatively to 178.8 degrees +/- 2.2 degrees postoperatively and the coronal femoro-tibial angle was adjusted on average 4.4 degrees. The measured preoperative tibial slope was 5.3 degrees +/- 2.2 degrees (mean +/- SD) and the average postoperative tibial slope was 4.7 degrees +/- 1.1 degrees on lateral views. The joint line was well preserved with an average modified Insall-Salvati index of 1.66 +/- 0.16 pre- and 1.67 +/- 0.16 postoperatively. The overall accuracy of fit of implant components was decent with a measured medial overhang of more than 1 mm (1.33 mm +/- 0.32 mm) in 4 cases only. Further, a lateral overhang of more than 1 mm (1.8 mm +/- 0.63) (measured in the anterior-posterior radiographs) was observed in 11 cases, with none of the 106 patients showing femoral notching.
Conclusion
The patient-specific iTotal\(^{TM}\) CR G2 total knee replacement system facilitated a proper fitting and positioning of the implant components. Moreover, a good restoration of the leg axis towards neutral alignment was achieved as planned. Nonetheless, further clinical follow-up studies are necessary to validate our findings and to determine the long-term impact of using this patient- specific system.
Auf der Suche nach neuen Vitamin D-responsiven Genen wurde die Methodik des Differential Display verwendet. Die eingesetzten Oligonukleotid-Primer waren homolog zur 24-Hydroxylase, einem für den Vitamin D-Stoffwechsel wichtigen Enzym, das von Vitamin D selbst reguliert wird. Mit diesem Ansatz sollten neue Mitglieder aus der Familie der P450-Enzyme gefunden werden. Mehrere differentiell exprimierte DNA-Fragmente wurden in der Folge isoliert und aufgearbeitet, die Sequenzierung eines 550 bp großen Fragments ergab beim Datenbankabgleich keine signifikanten Homologien und ließ daher auf ein noch unbekanntes Gen schließen. Die Klonierung und weitere Charakterisierung dieses Gens wies letztlich in eine nicht erwartete Richtung. Mit der Puromycin-sensitiven Aminopeptidase war ein Gen aus der Familie der Metallopeptidasen gefunden worden, eine Exopeptidase mit einem Zink-Ion im katalytischen Zentrum. Proteinchemisch schon vor einigen Jahren isoliert, war über die PSA (Puromycin-sensitive Aminopeptidase) bekannt, daß sie N-terminale Aminosäuren hydrolysiert mit einer Spezifität für basisch / neutrales und hydrophobes Substrat und überwiegend im Cytoplasma lokalisiert ist. Namengebend ist die starke Hemmbarkeit der Enzymaktivität durch Puromycin. Vorstehendes summiert die wichtigsten biochemischen Eigenschaften des Enzyms, aus molekularbiologischer Sicht war zum Zeitpunkt der Aufnahme der Arbeiten jedoch noch wenig über die PSA bekannt. Dies hat sich in letzter Zeit geändert, während der Sequenzierarbeiten im Rahmen dieser Arbeit konnte eine schweizer Arbeitsgruppe die Klonierung und Sequenz-Charakterisierung der murinen und später humanen cDNA des Gens veröffentlichen. Mit den Ergebnissen dieser Arbeitsgruppe wie auch den im Zuge der vorliegenden Dissertation erarbeiteten Resultaten konnte das molekularbiologische Wissen um die PSA erheblich erweitert werden. So liegt mittlerweile die komplette Nukleotid- und Aminosäurensequenz des Gens vor, das Auffinden des Zink-Bindungsmotives HEXXH(X)18E erlaubte die Einordnung des Enzyms in die Familie M1 der Metallopeptidasen, weiterhin konnte das Gen auf dem Chromosom 17q21 lokalisiert werden. Weitere Daten zur Gewebe- und Zellinienexpression sind nun ebenfalls vorhanden, interessant hierbei neben der erwartet starken Expression im Gehirn das deutliche Vorhandensein in Hoden und Nebennierenrinde. Auch in Bezug auf die Regulation der PSA wurden neue Ergebnisse erzielt, wichtigstes Resultat im Rahmen dieser Arbeit ist hier, daß sich die früher festgestellte Vitamin D –Responsivität nicht bestätigen ließ. Für die Zukunft von außerordentlichem Interesse dürfte das kürzlich gelungene Erstellen PSA-defizienter Mäuse sein, das nun erste Beobachtungen von Auswirkungen der Gen-Deletion am lebenden Organismus erlaubt. Trotz alledem sind in Bezug auf die physiologische Rolle der Puromycin-sensitiven Aminopeptidase weiterhin viele Fragen ungeklärt. Wie hoffentlich in dieser Dissertation herausgearbeitet, ergeben sich dabei interessante Perspektiven für die mögliche Funktion des Enzyms im Organismus.
Im ersten Teil dieser Arbeit wurden humane mesenchymale Stammzellen aus dem Knochenmark (mhMSCs) und aus trabekulären Knochenfragmenten (bhMSCs) isoliert und in einem neurogenen Prädifferenzierungsmedium mit BME gefolgt von einem Differenzierungsmedium mit BHA und DMSO kultiviert. Anschließend wurden die differenzierten hMSCs mit den jeweiligen undifferenzierten, im normalen Wachstumsmedium kultivierten, mhMSCs bzw. bhMSCs verglichen. Im Verlauf der 6-tägigen neurogenen Differenzierung zeigte sich eine deutliche Veränderung der Zellmorphologie. Die meisten differenzierten Zellen erschienen kleiner und bildeten lange, für Nervenzellen typische Zellfortsätze aus, die sich teilweise mehrfach verzweigten. Weiterhin fand sich bei den differenzierten mhMSCs und bhMSCs eine Expressionszunahme bzw. eine de novo Expression der neuronalen Marker NSE und tau auf Gen- bzw. Protein-Ebene. Es konnte jedoch keiner Expressionsänderung von NF-M auf Protein-Ebene und in mehr als der Hälfte der Fälle sogar einer Expressionsabnahme auf RNA-Ebene gefunden werden. Die Differenzierung zeigte keinen reproduzierbaren Effekt auf die Expression des Astrozyten-Markers GFAP. Diese Ergebnisse zeigten sich sowohl bei den mhMSCs als auch bei den bhMSCs und lassen vermuten, dass bhMSCs ein ähnliches Potential besitzen wie mhMSCs und nicht wie ursprünglich vermutet, auf die Differenzierung in Zellen mesenchymalen Ursprungs beschränkt sind. Die Tatsache, dass bereits undifferenzierte hMSCs neurogliale Marker (NF-M, NSE, GFAP) exprimieren konnte in dieser Arbeit für mhMSCs bestätigt und erstmals auch für bhMSC beobachtet werden. Diese Ergebnisse unterstützen die Vermutung, dass es sich bei mhMSCs und bhMSCs um Neuroglia-Vorläuferzellen handelt, die in der Lage sind in neuronale Zellen (Nerven- und Gliazellen) zu differenzieren, und dass die neurogene Differenzierung eher eine quantitative Modulation der Genexpression als ein einfaches An-/Abschalten Neuronen-spezifischer Gene bewirkt. Im zweiten Teil der Arbeit wurden zunächst Zellen vom peripheren Nerven isoliert und anschließend charakterisiert. Die aus dem peripheren Nerven herausgewachsenen spindelförmigen bzw. multipolaren Zellen bildeten mit ihren zahlreichen langen Fortsätzen ein regelrechtes Netzwerk und zeigten eine Expression der für Myelinmarker MPZ und PMP 22 sowie des Schwann-Zell-Markers S 100. Abschließend wurden mhMSCs und bhMSCs für 6 Tage in einem Schwann-Zell-Differenzierungsmedium mit Forskolin kultiviert. Die differenzierten spindelförmigen Zellen zeigten eine für Schwann-Zellen typischen fischzugartige Ausrichtung. Es fand sich eine starke Expressionszunahme der von den undifferenzierten Zellen nur schwach exprimierten Myelinmarker MPZ und PMP 22 auf RNA-Ebene sowie eine kräftige Expressionszunahme bzw. de novo Expression des Schwann-Zell-Markers S 100. Diese Ergebnisse zeigen, dass bhMSCs ebenso wie mhMSCS unter geeigneten Bedingungen in der Lage sind, eine für Schwann-Zellen typische Morphologie zu entwickeln und die Expression einzelner Schwann-Zell-Marker zu steigern. In wie weit sie auch funktionell Schwann-Zellen gleichen und damit entscheidend zur peripheren Nervenregeneration beitragen können, bleibt jedoch noch zu klären.
Die Arbeit untersucht Zusammenhänge zwischen der Veränderung geometrischer Parameter (gemessen an Röntgenaufnahmen) des Hüftgelenks durch den endoprothetischen Eingriff (primäre zementfreien Hüft-TEP) und dem patientenberichteten Outcome (EuroQol und WOMAC-Scores prä- sowie ein Jahr postoperativ) zu untersuchen. Hierzu wurden 655 Patienten (w: 336, m: 319) eingeschlossen (OP- Zeitraum von 2012–2014).
Im Mittel wurde der globale Offset (GO) um -1,14mm verringert (Vergrößerung des femoralen Offset (FO) durch den Eingriff um +4,61 mm und Verminderung des acetabulären Offsets (AO) um -5,74 mm). Bei den Frauen zeigte sich eine Vergrößerung des GO um +0,27 mm (AO -5,08 mm und FO +5,37 mm), bei den Männer eine Verkleinerung des GO um -2,62 mm (AO -6,43 mm und FO 3,81 mm).
In der Gesamtgruppe zeigten die Parameter der horizontalen Achse AO und FO keine Korrelationen zum Outcome. Jedoch konnte für den GO eine Korrelation zum EuroQol nachgewiesen werden (r=0,097; p<0,05) nachgewiesen werden. In der Gruppe der Frauen konnte eine positive Korrelation zwischen dem AO und dem WOMAC-Gesamtscore (r= 0,132; p<0,05) sowie den Subscores Schmerz (r=0,113; p<0,05) und Funktion (r=0,125; p<0,05) nachgewiesen werden. Die Ergebnisse bestätigen den Vorteil einer Medialisierung der Pfanne.
Bei der Analyse der Parameter der vertikale Achse (vertikaler Offset (VO), Hüftlänge (HL), Hip Center Height (HCH)) ergab sich in der Gesamtgruppe eine negative Korrelation des VO mit den WOMAC-Gesamtscore-Werten (r=-0,109; p<0,01) und den Werten des Subscores WOMAC-Schwierigkeit (r=0,106; p<0,01). Bei der HCH zeigte sich eine schwach positive Korrelation mit dem Subscore WOMAC-Schwierigkeit (r=0,081; p<0,05) und mit dem EurQol (r=0,0117; p<0,01). In der Subgruppe der Frauen ergaben sich bei der HCH eine Korrelation mit dem WOMAC-Gesamtscore (r=0,109; p<0,05) und dem Subscore WOMAC-Funktion (r=0,125; p<0,05). Die HL korrelierte in der Gruppe der Männer negativ mit dem WOMAC-Steifheit (r= -0,137; p<0,05). Die Daten bezüglich der vertikalen Achse deuten darauf hin, dass eine Vergrößerung des vertikalen Offsets sinnvoll zu sein scheint.
Bezüglich der diagonalen Achse konnte für den diagonalen Offset (def. als Vektor von GO und BL) ein positiver Zusammenhang mit dem EuroQol (r=0,110; p<0,01) nachgewiesen werden.
Mesenchymale Stammzellen (MSC) stellen die Grundlage der Knochenformation dar, indem sie als multipotente Zellen in viele, für die Knochenhomöostase benötigte Zelltypen differenzieren können, wie z.B. Osteoblasten. Während der Alterung des Menschen kommt es zu einem Ungleichgewicht zwischen Knochenaufbau und Knochenabbau, resultierend in einer verringerten Knochenmasse. Noch ist unklar, ob MSC an dem verminderten Knochenaufbau direkt beteiligt sind, indem sie z.B.im Laufe der Zeit Funktionsstörungen akkumulieren oder in die Seneszenz eintreten, und somit nicht mehr als Stammzellpool für die Osteoblastendifferenzierung zur Verfügung stehen. In der vorliegenden Arbeit wurde das Genexpressionsmuster gealterter Zellen mittels Mikroarray-Analysen untersucht, um die Alters-bedingten Veränderungen detektieren zu können. Hierfür wurde ein in-vitro-Alterungsmodell von humanen MSC (hMSC) etabliert, um die seneszenten Zellen mit hMSC früher Kultivierungspassagen zu vergleichen. Auch Zellen aus Spendern hohen Alters wurden untersucht, um einen Vergleich zwischen ex-vivo- und in-vitro-gealterten hMSC anstellen zu können. Da Osteoporose eine polygenetische Erkrankung des gealterten Knochens darstellt, wurden auch mit hMSC aus Osteoporose-Patienten Genexpressionsanalysen durchgeführt. Die Mikroarray-Analysen und anschließende systembiologische Auswertung zeigten, dass in-vitro-gealterte, seneszente hMSC starke Veränderungen im Transkriptom aufweisen, die auf Defizite in der Proliferation, Differenzierungskapazität und Migration schließen lassen. Neben bekannten Markern für replikative Seneszenz konnten in hMSC auch neue detektiert werden, wie z.B. HELLS, POU5F1 (OCT4) und FGFR2, deren Expression mit der Seneszenz abnimmt, oder CDH1 und PSG5, deren Expression zunimmt. Gene für Akute-Phase-SAA wurden stark erhöht exprimiert vorgefunden. Bei der funktionellen Charakterisierung konnte jedoch gezeigt werden, dass SAA1 und SAA1 durch Stress induziert werden, der der Seneszenz vorausgeht, und dass sie die Mineralisierung bei der osteogenen Differenzierung von hMSC fördern. Akute-Phase-SAA könnten somit eine Verbindung zwischen Alterung bzw. Inflammation und extra-skelettaler Verkalkung darstellen, die im Alter häufig auftritt, z.B. in Form von Arteriosklerose. In-vivo-gealterte hMSC wiesen ebenfalls Defizite im Expressionsmuster von Proliferations- und Migrations- relevanten Genen auf. Des Weiteren konnten nur wenige Gemeinsamkeiten zwischen in-vivo-gealterten hMSC und in-vitro-gealterten hMSC festgestellt werden. Dies lässt vermuten, dass die in-vivo-Alterung nicht zwangsläufig zu seneszenten Stammzellen führt, da Alterung eines Organismus ein multizellulärer Prozess ist, der durch viele Faktoren beeinflusst wird, wie z.B. Akkumulation von Mutationen und Krebsabwehr. Auch osteoporotische hMSC wiesen Veränderungen im Genexpressionsmuster auf, die mit den Daten zur in-vivo-Alterung verglichen wurden, um die rein Alters-assoziierten Änderungen herausfiltern zu können. Die übrig gebliebenen Gene repräsentierten Veränderungen allein aufgrund der Krankheit. Osteoporose bewirkte somit distinkte Genexpressions-änderungen in hMSC, die auf Förderung der Osteoklastogenese und Defizite in Proliferation, Migration und Differenzierungskapazität schließen lassen. Es konnten vielversprechende Kandidaten-gene für osteoporotische hMSC gefunden werden. Die prämature Expression des WNT-Inhibitors SOST (Sclerostin) und die Überexpression des BMP-Signalweg-Inhibitors MAB21L2 deuten auf eine Autoinhibition der Stammzellen hin, die letztlich die gestörte Knochenformation bei Alters-assoziierter Osteoporose begründen könnte. Zusammenfassend zeigt die vorliegende Arbeit, dass intrinsische Defizite von Stammzellen an der Pathophysiologie von Alterung und Osteoporose beteiligt sind. Sie eröffnet tiefgreifende Einblicke in die systembiologischen Veränderungen in Stammzellen aufgrund von Alterung oder Osteoporose, und setzt somit einen soliden Grundstein für weiterführende Analysen.
Primary osteoporosis is an age-related disease characterized by an imbalance in bone homeostasis. While the resorptive aspect of the disease has been studied intensely, less is known about the anabolic part of the syndrome or presumptive deficiencies in bone regeneration. Multipotent mesenchymal stem cells (MSC) are the primary source of osteogenic regeneration. In the present study we aimed to unravel whether MSC biology is directly involved in the pathophysiology of the disease and therefore performed microarray analyses of hMSC of elderly patients (79-94 years old) suffering from osteoporosis (hMSC-OP). In comparison to age-matched controls we detected profound changes in the transcriptome in hMSC-OP, e.g. enhanced mRNA expression of known osteoporosis-associated genes (LRP5, RUNX2, COL1A1) and of genes involved in osteoclastogenesis (CSF1, PTH1R), but most notably of genes coding for inhibitors of WNT and BMP signaling, such as Sclerostin and MAB21L2. These candidate genes indicate intrinsic deficiencies in self-renewal and differentiation potential in osteoporotic stem cells. We also compared both hMSC-OP and non-osteoporotic hMSC-old of elderly donors to hMSC of similar to 30 years younger donors and found that the transcriptional changes acquired between the sixth and the ninth decade of life differed widely between osteoporotic and non-osteoporotic stem cells. In addition, we compared the osteoporotic transcriptome to long term-cultivated, senescent hMSC and detected some signs for pre-senescence in hMSC-OP. Our results suggest that in primary osteoporosis the transcriptomes of hMSC populations show distinct signatures and little overlap with non-osteoporotic aging, although we detected some hints for senescence-associated changes. While there are remarkable inter-individual variations as expected for polygenetic diseases, we could identify many susceptibility genes for osteoporosis known from genetic studies. We also found new candidates, e.g. MAB21L2, a novel repressor of BMP-induced transcription. Such transcriptional changes may reflect epigenetic changes, which are part of a specific osteoporosis-associated aging process.
Angiopoetin-like 4 (ANGPTL-4) ist ein Adipokin, das in der extrazellulären Matrix lokalisiert ist und das neben seinen Aufgaben im Fettstoffwechsel auch die Zellmigration, Zellinvasion und die Angioneogenese reguliert. Da es zusätzlich auch den Knochenabbau fördert, wirkt es als Tumorpromotor speziell für Knochentumore. Aufgrund der gesteigerten Expression in Tumorgewebe ist es potenzielles Ziel für molekulare Bildgebung. Mittels Expressionsanalysen auf mRNA- und Proteinebene sollte ein besseres Verständnis der Regulation von ANGPTL-4 erreicht werden. Dexamethason und die 9-cis-Retinsäure beeinflussten die Expression von ANGPTL-4 in MDA Zellen im positiven Sinne, wohingegen der Adenylatcyclase Aktivator Forskolin die Expression supprimierte. In MCF-7 Zellen wurde ANGPTL-4 durch den Phorbolester PMA und durch den Epidermalen Wachstumsfaktor (EGF) induziert. Eine Modulation von ANGPTL-4 könnte von klinischem Nutzen sein, speziell bei der Behandlung von Knochentumoren. Zusätzlich könnte die Trennschärfe von molekularen bildgebenden Verfahren gesteigert werden.
Die vorliegende Studie berichtet erstmals über den Vergleich einer uni- mit einer bikondylären Endoprothese zur Therapie des M. Ahlbäck im Spätstadium. Da diese Osteonekrose nur das mediale Kompartiment betrifft, liegt die Indikation für eine unikompartimentelle Prothese vor. Trotzdem wurde entsprechend den Erfahrungen der Operateure in unserer Klinik seit 1988 häufiger eine bikondyläre Prothese inmplantiert. In der der Literatur findet sich bis heute keine einheitliche Meineung zu diesem Thema und es gibt nur wenige Studien zum M. Ahlbäck, da diese Erkrankung sehr selten auftritt. Daher wurde diese Studie konzipiert, mit dem Ziel, die Ergebnisse der unikondylären gegenüber der bikondylären Prothese zu vergleichen. In diese Studie wurden 37 Patienten (39 Knie) mit der Diagnose M. Ahlbäck eingeschlossen. Die prä- und intraoperativen klinischen und röntgenologisch Daten wurden retrospektiv erfaßt. Daraufhin wurden die Patienten bei einem Follow up nach durchschnitllich 6,6 (0,75-16,75) Jahren nochmals klinisch und röntgenologisch untersucht. Aufgrund der geringen Inzidenz des M. Ahlbäck war die Studie auf eine kleine Fallzahl beschränkt. Zusammenfassend kann gesagt werden, daß in allen klinischen und röntgenologischen Beobachtungen sowie in der Analyse der berechneten Scores die bikondyläre Prothese der unikondylären überlegen war. Die Verankerung der unikondylären Prothese im femoralen Knochenlager scheint aufgrund der Nekrosezone beim M. Ahlbäck nicht ausreichend gewährleistet zu sein. Daher empfehlen wir die bevorzugte Implantation einer bikondylären Prothese gegenüber einem unikondylären Modell für die operative Therapie des M. Ahlbäck.
Das ACL ist eine der am häufigsten verletzten Strukturen des menschlichen Körpers bei Sportunfällen und die Anzahl der Rekonstruktionen wird weiterhin zunehmen. In den letzten Jahren wurden große Fortschritte in der Charakterisierung der biomechanischen Eigenschaften von Sehnen und Bändern erzielt und neue Ansätze auf dem Gebiet des Tissue Engineering eröffnen neue Möglichkeiten. In dieser Arbeit wurde ein ACL-Konstrukt aus biomaterialbasiertem Kollagen I entwickelt und getestet. Die gewonnenen Ergebnisse können als Grundlage für die Herstellung eines Kreuzbandkonstruktes verwendet werden.
In den letzten Jahrzehnten ist die Anzahl der degenerativen Gelenkerkrankungen wie die Coxarthrose und somit die Zahl der zu implantierenden Hüfttotalendoprothesen stark gestiegen. Ziel der vorliegenden Arbeit ist es, langfristige Ergebnisse zementierter Titanschaftprothesen in Bezug auf aseptische Lockerungen zu ermitteln. Von den in der Orthopädischen Universitätsklinik Würzburg implantierten Hüfttotalendoprothesen von Januar 1990 bis März 1992 konnten nach durchschnittlich 9 Jahren 110 Hüfttotalendoprothesen klinisch und radiologisch nachuntersucht werden. Zum Zeitpunkt der Kontrolluntersuchung hatten die Patienten ein mittleres Alter von 76 Jahren. In allen Fällen wurden Müller-Geradschaftprothesen mit einer Ti-6A1-7Nb-Legierung in matter Oberfläche, Biolox®-Keramik-Köpfe sowie Knochenzement Palacos-® verwendet. Es wurde bei 4 Hüft-TEPs wegen aseptischer Lockerung ein Prothesenwechsel durchgeführt. Das nachuntersuchte Patientengut wurde in 3 Gruppen eingeteilt. Gruppe A rekrutierte sich aus denjenigen Patienten, bei denen keine radiologischen Lockerungszeichen erkennbar waren. Die Patienten mit mehr als einem Lysesaum jedoch mit festem Sitz der Schaftprothese im Vergleich zu den postoperativen Röntgenbildern wurden der Gruppe B zugeordnet. Letztendlich bildeten die Patienten mit ausgeprägten Lockerungszeichen bzw. vollständig gelockerte Prothesen die Gruppe C. Der Harris-Hip-Score der Gruppe A mit 85 ( 13) Punkten und der Gruppe B mit 86 ( 14) Punkten zeigte gute Ergebnisse. Der Harris-Score lag in der Gruppe C bei 76 (± 5) Punkten und erreichte somit ein mäßiges Ergebnis. Im Vergleich zu den Gruppen A und B erwies sich diese Punktezahl als signifikant schlechter. Die Patienten mit ausgeprägten Lockerungszeichen waren signifikant jünger (im Mittel 6 Jahre) als die der Gruppe ohne Lockerungssäume. Ebenfalls fanden wir einen signifikanten Unterschied im Bezug auf das Körpergewicht, Körpergewicht im Verhältnis zu zementierter Schaftoberfläche und Harris-Score (88 vs. 75 kg; 1,5 vs. 1,0 kg/cm; 76 vs. 85). Für Geschlecht, Schaftgröße, Schaftart, Aktivität, heterotope Ossifikationen und Body-Maß-Index traf dies nicht zu. Unter Berücksichtigung der erhobenen Daten (Harris-Hüft-Score und Quotient des Körpergewichts zur zementierten Schaftoberfläche) sollte eine möglichst große Prothese implantiert werden, um das Körpergewicht auf eine große Schaftoberfläche zu verteilen. Insgesamt hat sich die zementierte Müller-Geradschaftprothese aus Titanlegierung bewährt, so dass sie für die Behandlung von Nickelallergiker zu empfehlen ist.
