In der Verlaufskontrolle von Patienten mit Adipositas erleichtern zuverlässige und einfach durchführbare Methoden zur Untersuchung der Körperzusammensetzung die Beurteilung des Ernährungszustandes. Hierzu bieten sich anthropometrische Verfahren sowie die Bioelektrische Impedanzmessung an. Patienten mit Kraniopharyngeom und gesunde Kinder und Jugendliche wurden diesbezüglich untersucht. 344 gesunde Probanden und 26 Patienten mit Kraniopharyngeom wurden untersucht hinsichtlich Impedanz, BMI und Hautfaltendicke. Die Fettfreie Masse bzw. der Körperfettanteil wurde nach Formeln errechnet. Als Referenzmethode diente für die Kraniopharyngeompatienten die Zwei-Energie-Röntgenabsorptiometrie (DXA). Anhand der Daten von 344 gesunden Kindern und Jugendlichen wurden Normalwerte für BI, BMI und Hautfaltendicken erarbeitet. Es ließ sich ein statistisch signifikanter Unterschied der beiden Geschlechter im Hinblick auf die gemessene Hautfaltendicke, die Impedanz und den aus der Impedanz errechneten Körperfettanteil zeigen. Die Anwendung publizierter Formeln erscheint nur für das Normalkollektiv geeignet. Auch für junge Kraniopharyngeompatienten erwiesen sich Hautfaltenmessung und Bioelektrische Impedanzmessung als einfache und kostengünstige Bedsite-Methoden geeignet zur Bestimmung der Körperzusammensetzung. Dies wurde durch die hohe Korrelation zu den Messwerten aus der DXA-Messung bestätigt. Resistance-Index (Körperhöhe²/Impedanz), BMI und die Subscapularfalte waren gleichwertig in ihrer hohen Korrelation zur FFM bzw. Fettmasse (FM). Unter Verwendung einer neuen Berechnungsformel bietet die Impedanzmessung eine einfache und zuverlässige Methode zur Bestimmung der Körperzusammensetzung bei Patienten mit Kraniopharyngeom. Die Verlaufsbeurteilung der Körperzusammensetzung mittels BI wird prospektiv in der multizentrischen Beobachtungsstudie Kraniopharyngeom 2000 ausgewertet.
We report on a nine-month-old girl who presented with persisting cough, and diminished ventilation of the left hemithorax. Viral pneumonia was suspected after Adenovirus detection by PCR, but chest X-rays showed a persistent shadowing of the left hemithorax and persistent coughing despite clinical improvement. Because of the discrepancy between clinical and radiological signs further investigations by ultrasound and CT scan were performed, which visualized an intrathroracic tumor. Histopathology confirmed diagnosis of a teratoma.
This case highlights the need for careful evaluation by the treating physicians. If the chest X-ray provides a discrepancy to the clinical findings or persistent pathologies exist, differential diagnosis should be discussed and further diagnostics be performed.
ESPED survey on newly diagnosed immune thrombocytopenia in childhood: how much treatment do we give?

Background
Immune thrombocytopenia (ITP) is an autoimmune disease associated with isolated thrombocytopenia, which is caused by an imbalance between platelet production and platelet destruction. Petechial and mucous membrane hemorrhages are characteristic of ITP, but life-threatening bleeding rarely occurs. Depending on the bleeding symptoms, ITP can be treated with glucocorticoids (GC), intravenous immunoglobulins (IVIG), or in severe cases, platelet transfusions. Mild bleeding does not necessarily require therapy. Using the German Surveillance Unit for rare Pediatric Diseases (ESPED) we conducted a prospective survey on ITP patients in all German Children's Hospitals between September 2018 and August 2019. We collected data on ITP, including the clinical course, therapy implementation recommendations (according to the Association of German Scientific Medical Societies guidelines), outcome, and influence of treatment regimens depending on the treating physician´s experience with ITP patients.
Results
Of the 287 recorded cases of children with ITP, 268 questionnaires were sent to the authors. Two hundred seventeen of the questionnaires fulfilled the inclusion criteria. ITP affected boys and girls similarly, and the median age of manifestation was 3.5 years. The main reasons for hospitalization were thrombocytopenia, bleeding signs, hematomas, and/or petechiae. Bleeding scores were ≤ 3 in 96% of children, which corresponded to a low-to-moderately low risk of bleeding. No life-threatening bleeding was documented. The most common therapies were IVIG (n = 59), GC (n = 33), or a combination of these (n = 17). Blood products (i.e., red blood cells, platelet concentrate, and fresh frozen plasma) were given to 13 patients. Compared to the established guidelines, 67 patients were over-treated, and 2 patients were under-treated.
Conclusions
Adherence to German ITP treatment guidelines is currently limited. To improve patient safety and medical care, better medical training and dissemination of the guidelines are required in line with targeted analyses of patients with serious bleeding events to identify potential risk constellations.
Mitte des 19. Jahrhunderts gab es in Deutschland im Vergleich zu vielen seiner Nachbarstaaten nur sehr wenige Heil- und Krankenanstalten speziell für Kinder. Die Anerkennung der Kinderheilkunde als eigenes Fachgebiet an den deutschen Universitäten verlief sehr langsam. Untersucht man die historische und soziologische Situation in Deutschland sowie den Entwicklungsstand der Medizin im 19. Jahrhundert genauer, so wird deutlich, warum hier solche aus heutiger Sicht rückständige Verhältnisse herrschten. Wie sich anhand von Originaldokumenten belegen lässt, wurde in Würzburg für deutsche Verhältnisse recht früh der Vorstoß unternommen, an der Universität eine "Separatanstalt" für kranke Kinder zu errichten und die Pädiatrie als eigenes Lehrfach einzuführen. Am 7. Juli 1841 erreichte den Senat und die Fakultät der Universität Würzburg ein Königlich Bayerisches Dekret, das die Errichtung einer klinischen Anstalt für Kinder und eine damit verbundene Abhaltung von Vorlesungen über Kinderkrankheiten vorsah. Das primäre Vorhaben der Fakultät, eine "stabile" Kinderklinik zu eröffnen, wurde jedoch aufgrund von Finanzierungsproblemen für mehrere Jahre aufgeschoben. Stattdessen wurde 1844 eine "billigere" ambulante Kinderklinik eröffnet und Franz von Rinecker, ein sehr engagierter Professor, zu deren Leiter und zum Prof. für Kinderkrankheiten ernannt. Erst am 1. Nov. 1850 kam es zur offiziellen Gründung einer stabilen Kinderklinik mit 12-15 Betten. Da andieser Institution jedoch sowohl die Universität als auch das Juliusspital, eine sehr mächtige und mit der Universität verschwisterte Stiftung, beteiligt waren, kam es sowohl vor als auch nach der Gründung zu großen Interessenskonflikten und Machtkämpfen.Das Resultat war, dass die Universität und der von ihr ernannte Vorstand, Franz von Rinecker, letztendlich nur einen sehr geringen Einfluss in der Kinderklinik hatten, die außerdem bereits 1872 wieder ihre Eigenständigkeit verlor. Die "stabile" Kinderklinik wurde an die Innere Medizin abgetreten, die ambulante Kinderklinik fiel an die allgemeine Poliklinik zurück. Erst 1915, also über 40 Jahre später, erlangte die Würzburger Pädiatrie wieder ihre Eigenständigkeit, die mit dem Ordinariat für Hans Rietschel ab 1923 vervollständigt wurde. Das Schicksal der pädiatrischen Anstalten in Würzburg ist bezeichnend für die damalige Zeit: Die deutsche Pädiatrie steckte noch in ihren Kinderschuhen und befand sich im ständigen Kampf gegen die Dominanz von Allgemeiner Poliklinik und Innerer Medizin.
Background
Influenza virus infections in immunologically naïve children (primary infection) may be more severe than in children with re-infections who are already immunologically primed. We compared frequency and severity of influenza virus primary and re-infections in pre-school children requiring outpatient treatment.
Methods
Influenza-unvaccinated children 1–5 years of age presenting at pediatric practices with febrile acute respiratory infection < 48 h after symptom onset were enrolled in a prospective, cross-sectional, multicenter surveillance study (2013–2015). Influenza types/subtypes were PCR-confirmed from oropharyngeal swabs. Influenza type/subtype-specific IgG antibodies serving as surrogate markers for immunological priming were determined using ELISA/hemagglutination inhibition assays. The acute influenza disease was defined as primary infection/re-infection by the absence/presence of influenza type-specific immunoglobulin G (IgG) and, in a second approach, by the absence/presence of subtype-specific IgG. Socio-demographic and clinical data were also recorded.
Results
Of 217 influenza infections, 178 were due to influenza A (87 [49%] primary infections, 91 [51%] re-infections) and 39 were due to influenza B (38 [97%] primary infections, one [3%] re-infection). Children with “influenza A primary infections” showed fever with respiratory symptoms for a shorter period than children with “influenza A re-infections” (median 3 vs. 4 days; age-adjusted p = 0.03); other disease characteristics were similar. If primary infections and re-infections were defined based on influenza A subtypes, 122 (87%) primary infections (78 “A(H3N2) primary infections”, 44 “A(H1N1)pdm09 primary infections”) and 18 (13%) re-infections could be classified (14 “A(H3N2) re-infections” and 4 “A(H1N1)pdm09 re-infections”). Per subtype, primary infections and re-infections were of similar disease severity. Children with re-infections defined on the subtype level usually had non-protective IgG titers against the subtype of their acute infection (16 of 18; 89%). Some patients infected by one of the influenza A subtypes showed protective IgG titers (≥ 1:40) against the other influenza A subtype (32/140; 23%).
Conclusions
Pre-school children with acute influenza A primary infections and re-infections presented with similar frequency in pediatric practices. Contrary to expectation, severity of acute “influenza A primary infections” and “influenza A re-infections” were similar. Most “influenza A re-infections” defined on the type level turned out to be primary infections when defined based on the subtype. On the subtype level, re-infections were rare and of similar disease severity as primary infections of the same subtype. Subtype level re-infections were usually associated with low IgG levels for the specific subtype of the acute infection, suggesting only short-time humoral immunity induced by previous infection by this subtype. Overall, the results indicated recurring influenza virus infections in this age group and no or only limited heterosubtypic antibody-mediated cross-protection.
Background
Previous influenza surveillance at paediatric intensive care units (PICUs) in Germany indicated increased incidence of PICU admissions for the pandemic influenza subtype A(H1N1)pdm09. We investigated incidence and clinical characteristics of influenza in children admitted to PICUs during the first three post-pandemic influenza seasons, using active screening.
Methods
We conducted a prospective surveillance study in 24 PICUs in Bavaria (Germany) from October 2010 to September 2013. Influenza cases among children between 1 month and 16 years of age admitted to these PICUs with acute respiratory infection were confirmed by PCR analysis of respiratory secretions.
Results
A total of 24/7/20 influenza-associated PICU admissions were recorded in the post-pandemic seasons 1/2/3; incidence estimates per 100,000 children were 1.72/0.76/1.80, respectively. Of all 51 patients, 80 % had influenza A, including 65 % with A(H1N1)pdm09. Influenza A(H1N1)pdm09 was almost absent in season 2 (incidence 0.11), but dominated PICU admissions in seasons 1 (incidence 1.35) and 3 (incidence 1.17). Clinical data was available for 47 influenza patients; median age was 4.8 years (IQR 1.6–11.0). The most frequent diagnoses were influenza-associated pneumonia (62 %), bronchitis/bronchiolitis (32 %), secondary bacterial pneumonia (26 %), and ARDS (21 %). Thirty-six patients (77 %) had underlying medical conditions. Median duration of PICU stay was 3 days (IQR 1–11). Forty-seven per cent of patients received mechanical ventilation, and one patient (2 %) extracorporeal membrane oxygenation; 19 % were treated with oseltamivir. Five children (11 %) had pulmonary sequelae. Five children (11 %) died; all had underlying chronic conditions and were infected with A(H1N1)pdm09. In season 3, patients with A(H1N1)pdm09 were younger than in season 1 (p = 0.020), were diagnosed more often with bronchitis/bronchiolitis (p = 0.004), and were admitted to a PICU later after the onset of influenza symptoms (p = 0.041).
Conclusions
Active screening showed a continued high incidence of A(H1N1)pdm09-associated PICU admissions in the post-pandemic seasons 1 and 3, and indicated possible underestimation of incidence in previous German studies. The age shift of severe A(H1N1)pdm09 towards younger children may be explained by increasing immunity in the older paediatric population. The high proportion of patients with underlying chronic conditions indicates the importance of consistent implementation of the current influenza vaccination recommendations for risk groups in Germany.
Rapid immune reconstitution (IR) following stem cell transplantation (SCT) is essential for a favorable outcome. The optimization of graft composition should not only enable a sufficient IR but also improve graft vs. leukemia/tumor effects, overcome infectious complications and, finally, improve patient survival. Especially in haploidentical SCT, the optimization of graft composition is controversial. Therefore, we analyzed the influence of graft manipulation on IR in 40 patients with acute leukemia in remission. We examined the cell recovery post haploidentical SCT in patients receiving a CD34(+)-selected or CD3/CD19-depleted graft, considering the applied conditioning regimen. We used joint model analysis for overall survival (OS) and analyzed the dynamics of age-adjusted leukocytes; lymphocytes; monocytes; CD3(+), CD3(+) CD4(+), and CD3(+) CD8(+) T cells; natural killer (NK) cells; and B cells over the course of time after SCT. Lymphocytes, NK cells, and B cells expanded more rapidly after SCT with CD34(+)-selected grafts (P = 0.036, P = 0.002, and P < 0.001, respectively). Contrarily, CD3(+) CD4(+) helper T cells recovered delayer in the CD34 selected group (P = 0.026). Furthermore, reduced intensity conditioning facilitated faster immune recovery of lymphocytes and T cells and their subsets (P < 0.001). However, the immune recovery for NK cells and B cells was comparable for patients who received reduced-intensity or full preparative regimens. Dynamics of all cell types had a significant influence on OS, which did not differ between patients receiving CD34(+)-selected and those receiving CD3/CD19-depleted grafts. In conclusion, cell reconstitution dynamics showed complex diversity with regard to the graft manufacturing procedure and conditioning regimen.
Background: Routine annual influenza vaccination is primarily recommended for all persons aged 60 and above and for people with underlying chronic conditions in Germany. Other countries have already adopted additional childhood influenza immunisation programmes. The objective of this study is to determine the potential epidemiological impact of implementing paediatric influenza vaccination using intranasally administered live-attenuated influenza vaccine (LAIV) in Germany.
Methods: A deterministic age-structured model is used to simulate the population-level impact of different vaccination strategies on the transmission dynamics of seasonal influenza in Germany. In our base-case analysis, we estimate the effects of adding a LAIV-based immunisation programme targeting children 2 to 17 years of age to the existing influenza vaccination policy. The data used in the model is based on published evidence complemented by expert opinion.
Results: In our model, additional vaccination of children 2 to 17 years of age with LAIV leads to the prevention of 23.9 million influenza infections and nearly 16 million symptomatic influenza cases within 10 years. This reduction in burden of disease is not restricted to children. About one third of all adult cases can indirectly be prevented by LAIV immunisation of children.
Conclusions: Our results demonstrate that vaccinating children 2-17 years of age is likely associated with a significant reduction in the burden of paediatric influenza. Furthermore, annual routine childhood vaccination against seasonal influenza is expected to decrease the incidence of influenza among adults and older people due to indirect effects of herd protection. In summary, our model provides data supporting the introduction of a paediatric influenza immunisation programme in Germany.
Henoch-Schönlein Purpura (HSP) or IgA vasculitis is the most common systemic vasculitis of childhood and may affect skin, joints, gastrointestinal tract, and kidneys. Skin manifestations of HSP are characteristic and include a non-thrombocytopenic palpable purpura of the lower extremities and buttocks. Rarely, HSP may initially present as or evolve into hemorrhagic vesicles and bullae. We present an otherwise healthy 5-year-old boy with an acute papulovesicular rash of both legs and intermittent abdominal pain. After a few days the skin lesions rapidly evolved into palpable purpura and hemorrhagic bullous lesions of variable size and severe hemorrhagic HSP was suspected. A histological examination of a skin biopsy showed signs of a small vessel leukocytoclastic vasculitis limited to the upper dermis and direct immunofluorescence analysis revealed IgA deposits in vessel walls, compatible with HSP. To further characterize the clinical picture and treatment options of bullous HSP we performed an extensive literature research and identified 41 additional pediatric patients with bullous HSP. Two thirds of the reported patients were treated with systemic corticosteroids, however, up to 25% of the reported patients developed skin sequelae such as hyperpigmentation and/or scarring. The early use of systemic corticosteroids has been discussed controversially and suggested in some case series to be beneficial by reducing the extent of lesions and minimizing sequelae of disease. Our patient was treated with systemic corticosteroids tapered over 5 weeks. Fading of inflammation resulted in healing of most erosions, however, a deep necrosis developing from a large blister at the dorsum of the right foot persisted so that autologous skin transplantation was performed. Re-examination 11 months after disease onset showed complete clinical remission with re-epithelialization but also scarring of some affected areas.
In der Dissertation wurden die Daten von 645 Kindern und Jugendlichen in Deutschland mit Pleuraempyemen oder parapneumonischen Ergüssen (PE/PPE) analysiert, welche im Zeitraum von Oktober 2010 bis Juni 2013 in deutschen Kinderkliniken stationär aufgenommen wurden.
Schwerpunkte der Arbeit waren die Erfassung und Analyse der vorkommenden Erreger, der Pneumokokkenserotypen und des Pneumokokkenimpfstatus, sowie der antibiotischen Therapie
203 von 645 Kindern und Jugendlichen mit PE/PPE wiesen einen positiven Erregernachweis in der Blutkultur, der Pleurapunktatkultur und/oder der Pleurapunktat-PCR auf. Der häufigste vorkommende Erreger war mit 55% S. pneumoniae. S. pyogenes stellte mit 15% den zweithäufigsten Erreger dar. S. epidermidis machte 4% und S. aureus 3% der nachgewiesenen Erreger aus. Bei allen drei Nachweismethoden (Blutkultur, Pleurapunktatkultur und Pleurapunktat-PCR) war einzeln betrachtet S. pneumoniae jeweils der häufigste nachgewiesene Erreger.
Beim Vergleich von Patienten mit positivem Erregernachweis in Blutkultur, Pleurapunktatkultur oder Pleurapunktat-PCR mit Patienten ohne Erregernachweis, zeigten die Patienten mit positivem Erregernachweis eine längere Krankenhausaufenthaltsdauer (19 vs. 16 Tagen im Median, p-value <0,001), eine höhere Komplikationsrate (80% vs. 57%, p-value <0,001)) sowie eine häufigere Eröffnung des Pleuraraumes (94% vs. 71%, p-value<0,001). Es kam bei Patienten mit positivem Erregernachweis ebenso häufiger zu einer Intensivpflichtigkeit (74% vs. 51%, p-value<0,001), sowie zu gesicherten oder möglichen Krankheitsfolgen (25% vs. 15%, p-value 0,004).
Vergleicht man, bei Patienten mit positivem Erregernachweis, die unterschiedlichen Erreger (S. pneumoniae, S. pyogenes, S. epidermidis, S. aureus, „andere Erreger“) hinsichtlich der klinischen Charakteristika, so zeigen sich keine wesentlichen Unterschiede bezüglich des klinischen Verlaufes, sowie der Akut- und Langzeit-Komplikationen.
Die Serotypen des am häufigsten aufgetretenen Erregers S. pneumoniae wurden molekularbiologisch identifiziert. Insgesamt konnte bei 36% der Patienten mit S. pneumoniae der Pneumokokkenserotyp nachgewiesen werden. Der häufigste Serotyp war Serotyp 1, der zweithäufigste Serotyp 3. Diese beiden Serotypen sind nicht im Pneumokokken-Konjugatimpfstoff PCV-7, jedoch im Pneumokokken-Konjugatimpfstoff PCV-13 enthalten. Es wurde nur ein Serotyp (35F) nachgewiesen, welcher in keinem der derzeit zugelassenen polyvalenten Konjugatimpfstoffen enthalten ist.
Bei der Betrachtung des Pneumokokkenimpfstatus der Kinder und Jugendlichen mit PE/PPE zeigte sich, dass 60% der Patienten (294 von 490 Patienten mit bekanntem Pneumokokkenimpfstatus) mit mindestens einer Dosis Pneumokokkenimpfstoff geimpft worden waren. Der am häufigsten verwendete Impfstoff war der Pneumokokken-Konjugatimpfstoff PCV-7, der zweithäufigste der Pneumokokken-Konjugatimpfstoff PCV-13.
Bei 6 der geimpften Patienten wurde ein Pneumokokkenserotyp nachgewiesen, welcher in dem mindestens einmal geimpften Pneumokokkenimpfstoff enthalten war. Dabei wurde bei 2 von den 6 Patienten mit Durchbruchsinfektion der Serotyp 3 nachgewiesen.
Die verwendeten Antibiotika bei den Kindern und Jugendlichen mit PE/PPE wurden genauer analysiert. 35% der Patienten erhielten eine vorstationäre Antibiotikatherapie. Am häufigsten wurden dabei Cephalosporine eingesetzt. Patienten, welche vorstationär Antibiotika erhalten haben, hatten eine kürzere Krankenhausaufenthaltsdauer (16 vs. 18 Tage im Median, p-value 0,026), eine geringere Wahrscheinlichkeit für eine Intensivpflichtigkeit (51% vs. 62%, p-value 0,009), jedoch eine längere Dauer der vorstationären Erkrankung (7 vs. 4 Tage im Median, p-value <0,001) bei jeweils gleicher Gesamtdauer des Pleuraergusses (14 Tage im Median). Außerdem war die Nachweiswahrscheinlichkeit eines Erregers in Blutkultur, Pleurapunktatkultur und/oder Pleurapunktat-PCR bei Patienten mit vorstationärer Antibiotikagabe geringer (26% vs. 35%, p-value 0,024) und es gab Unterschiede in der Erregerverteilung zwischen Patienten mit und ohne vorstationärer Antibiotikagabe. So machte S. pneumoniae bei Patienten mit vorstationärer Antibiotikagabe 41% der Erreger aus, bei Patienten ohne vorstationäre Antibiotikagabe 61%. Bei Patienten mit vorstationärer Antibiotikagabe zeigte sich dafür ein höherer Anteil von 37% der Gruppe der „anderen Erreger“ (welche nicht zu den vier häufigsten Erregern S. pneumoniae, S. pyogenes, S. epidermidis und S. aureus gehören), als bei Patienten ohne vorstationäre Antibiotikatherapie. Bei Patienten ohne vorstationäre Antibiotikagabe machten die „anderen Erreger“ lediglich 16% der Erreger aus.
Stationär erhielten 99% der Patienten eine intravenöse Therapie und 45% der Patienten orale Antibiotika. Am häufigsten wurden intravenös Cephalosporine der 2. Generation, wie beispielsweise Cefuroxim, verabreicht. Oral wurden stationär am häufigsten Makrolide, zum Beispiel Erythromycin oder Clarithromycin, eingesetzt.
Der relativ häufige Einsatz von Makroliden (59% der stationär eingesetzten oralen Antibiotika sowie 26% der vorstationären Antibiotika) ist bei nicht optimaler Wirksamkeit und hoher Resistenzrate von S. pneumoniae gegenüber Makroliden bei Kindern (Imöhl et al. 2010) kritisch zu betrachten.
Bei parapneumonischen Ergüssen, bzw. Pleuraempyemen, handelt es sich um eine schwere Erkrankung im Kindes- und Jugendalter, deren häufigster Erreger S. pneumoniae ist. Die zwischen Oktober 2010 und Juni 2013 gefundenen Pneumokokkenserotypen waren größtenteils nicht in dem, zwischen 2006 und 2009 überwiegend verwendeten, 7-valenten Pneumokokkenkonjugatimpfstoff enthalten, während Pneumokokkenserotypen, welche im seit 2009 überwiegend verwendeten 13-valenten Pneumokokkenkonjugatimpfstoff enthalten sind, vorherrschten. Damit besteht aktuell eine gute Möglichkeit der Impfprävention gegenüber dieser schweren Komplikation der ambulant erworbenen Pneumonie. Die Wirksamkeit gegenüber dem prinzipiell durch den 13-valenten Impfstoff erfassten Pneumokokken-Serotyp 3, bei dem in der vorliegenden Erhebung 2 Durchbruchsinfektionen beobachtet wurden, erscheint jedoch möglicherweise als nicht ausreichend. In dem hier betrachteten Zeitraum von Oktober 2010 bis Juni 2013 kam es nicht zu einer Zunahme der Krankenhausaufnahmen aufgrund von PE/PPE bei Kindern und Jugendlichen. Dies steht im Gegensatz zu Studien aus anderen Ländern, welche auf einen Anstieg der Pleuraempyeminzidenz bei Kindern hinweisen (Hendrickson et al. 2008; Byington et al. 2006; Sakran et al. 2014). Eine weitere Surveillance der Inzidenz und verursachenden Erreger von parapneumonischen Ergüssen und Pleuraempyemen im Kindesalter ist daher, insbesondere bezüglich eines möglichen Serotypenreplacements oder einer Erregerverschiebung, notwendig und damit auch für die Impfprävention von hoher Bedeutung.
Background
Increasing bacterial resistance to antibiotics is a serious problem worldwide. We sought to record the acquisition of antibiotic-resistant Escherichia coli (E. coli) in healthy infants in Northern Thailand and investigated potential determinants.
Methods
Stool samples from 142 infants after birth, at ages 2wk, 2mo, 4 to 6mo, and 1y, and parent stool samples were screened for E. coli resistance to tetracycline, ampicillin, co-trimoxazole, and cefazoline by culture, and isolates were further investigated for multiresistance by disc diffusion method. Pulsed-field gel electrophoresis was performed to identify persistent and transmitted strains. Genetic comparison of resistant and transmitted strains was done by multilocus sequence typing (MLST) and strains were further investigated for extra- and intra-intestinal virulence factors by multiplex PCR.
Results
Forty-seven (33%) neonatal meconium samples contained resistant E. coli. Prevalence increased continuously: After 1y, resistance proportion (tetracycline 80%, ampicillin 72%, co-trimoxazole 66%, cefazoline 35%) almost matched those in parents. In 8 infants (6%), identical E. coli strains were found in at least 3 sampling time points (suggesting persistence). Transmission of resistant E. coli from parents to child was observed in only 8 families. MLST showed high diversity. We could not identify any virulence genes or factors associated with persistence, or transmission of resistant E. coli. Full-term, vaginal birth and birth in rural hospital were identified as risk factors for early childhood colonization with resistant E. coli.
Conclusion
One third of healthy Thai neonates harboured antibiotic-resistant E. coli in meconium. The proportion of resistant E. coli increased during the first year of life almost reaching the value in adults. We hypothesize that enhancement of infection control measures and cautious use of antibiotics may help to control further increase of resistance.
Der klinische Stellenwert der Polysomnographie bei Kindern mit kraniofazialen Fehlbildungssyndromen

Schlaf- und nächtliche Atemstörungen sind ein häufiges Symptom bei Kindern mit kraniofazialen Fehlbildungen, da es durch die Mittelgesichtshypoplasie zu einer Einengung der Atemwege kommen kann. Wir haben bei 28 Kindern Polysomnographien vorgenommen, um differenziertere Aussagen über die Art der schlafbezogenen Apnoen, den Zusammenhang mit bestimmten Schlafstadien und Schlafarchitektur und Korrelationen mit der mental-kognitiven Entwicklung sowie der Lebensqualität machen zu können. Diese wurden mittels dreier Fragebögen und psychologischer Testung bestimmt. Bei 5 Patienten fand sich ein schweres, bei 9 ein mäßiges und bei 12 ein mildes obstruktives Schlaf-Apnoe-Syndrom, nur 2 Kinder hatten kein OSAS. Die Kinder mit Crouzon-Syndrom hatten überwiegend (9/14) ein mittel- bis schwergradiges OSAS, während die Kinder mit Apert-Syndrom mehrheitlich von einer leichten Atemstörung betroffen waren. Neben obstruktiven Ereignissen war auch die Zahl der zentralen Apnoen erhöht. Aufgrund der hohen Korrelationen zwischen OSAS-Schweregrad und Entsättigungsindex könnte die Pulsoxymetrie als ambulantes Screeningverfahren eingesetzt werden. Die Schlafarchitektur war nicht wesentlich beeinträchtigt bei einer eindrucksvollen Häufung der respiratorischen Ereignisse im REM-Schlaf (RDI 16,97/h in REM vs 8,2 in NREM) und einem in Abhängigkeit vom Schweregrad des OSAS erhöhten Arousalindex. Insbesondere nasaler CPAP und v. a. BiPAP, aber auch Adenotonsillektomien, führten zu einer deutlichen Besserung der respiratorischen Parameter. Aufgrund der Dynamik der Erkrankung sind engmaschige Kontrollen und ggf. Erweiterungen und Anpassungen der Therapie nötig. Die Lebensqualität lag insgesamt im Normbereich - unabhängig von der OSAS-Schwere - zeigte jedoch im Verlauf einen Zusammenhang mit dieser und der Klinik. Obwohl keine signifikanten Korrelationen zwischen Tagessymptomatik, einzelnen PSG-Parametern und OSAS-Schwere aufzuzeigen war, spiegelte sich in den Fragebögen eine erfolgreiche Therapie wider, so dass ein überarbeiteter Fragebogen als Therapie- und Verlaufskontrolle dienen könnte.
Background: IL-12p40 plays an important role in the activation of the T-cell lines like Th17 and Th1-cells. Theses cells are crucial in the pathogenesis of juvenile idiopathic arthritis. A polymorphism in its promoter region and the genotype IL12p40 pro1.1 leads to a higher production of IL-12p40. We studied whether there is a difference in the distribution of the genotype in patients with JIA and the healthy population.
Methods: In 883 patients and 321 healthy controls the IL-12p40 promoter genotype was identified by ARMS-PCR.
Results: There is no association of IL-12p40 pro polymorphism neither in patients with JIA compared to controls nor in subtypes of JIA compared to oligoarthritis. We found a non-significant tendency of a higher prevalence of the genotype pro1.1 in systemic arthritis (32.4 %) and in rheumatoid factor negative polyarthritis (30.5 %) and a lower pro1.1 genotype in persistent oligoarthritis (20.7 %) and in enthesitis-related arthritis (17 %). Likelihood of the occurrence of genotype IL12-p40 pro1.1 in patients with systemic arthritis (OR 1.722, CI 95 % 1.344-2.615, p 0.0129) and RF-negative polyarthritis (OR 1.576, CI 95 % 1.046-2.376, p 0.0367) compared to persistent oligoarthritis was significantly higher. This was also true for comparison of their homozygous genotypes IL-12p40 pro 1.1 and 2.2 in systemic arthritis (OR 1.779, CI 95 % 1.045-3.029, p 0.0338). However, in Bonferroni correction for multiple hypothesis this was not significant.
Conclusion: A tendency of a higher prevalence of the genotype IL-12p40 pro1.1 in systemic arthritis and in rheumatoid factor negative polyarthritis was observed but not significant. Further investigations should be done to clarify the role IL-12p40 in the different subtypes of JIA.
Background
Medulloblastoma is the most common malignant brain tumor in children and can be divided in different molecular subgroups. Patients whose tumor is classified as a Group 3 tumor have a dismal prognosis. However only very few tumor models are available for this subgroup.
Methods
We established a robust orthotopic xenograft model with a cell line derived from the malignant pleural effusions of a child suffering from a Group 3 medulloblastoma.
Results
Besides classical characteristics of this tumor subgroup, the cells display cancer stem cell characteristics including neurosphere formation, multilineage differentiation, CD133/CD15 expression, high ALDH-activity and high tumorigenicity in immunocompromised mice with xenografts exactly recapitulating the original tumor architecture.
Conclusions
This model using unmanipulated, human medulloblastoma cells will enable translational research, specifically focused on Group 3 medulloblastoma.
Background
Children with different underlying malignant diseases require long-term central venous access. As for port systems in a pectoral position, peripherally implanted port systems in the forearm revealed high levels of technical and clinical success in adult cohorts.
Objective
To investigate the technical and clinical outcomes of percutaneous central venous port implantation in the forearm in adolescents.
Materials and methods
Between April 2010 and August 2020, 32 children ages 9 to 17 years with underlying malignancy received 35 totally implantable venous access ports (TIVAPs) in the forearm. All venous port systems were peripherally inserted under ultrasound guidance. Correct catheter placement was controlled by fluoroscopy. As primary endpoints, the technical success, rate of complications and catheter maintenance were analyzed. Secondary endpoints were the side of implantation, vein of catheter access, laboratory results on the day of the procedure, procedural radiation exposure, amount of contrast agent and reasons for port device removal.
Results
Percutaneous TIVAP placement under sonographic guidance was technically successful in 34 of 35 procedures (97.1%). Procedure-related complications did not occur. During the follow-up, 13,684 catheter days were analyzed, revealing 11 complications (0.8 per 1,000 catheter-duration days), Of these 11 complications, 7 were major and 10 occurred late. In seven cases, the port device had to be removed; removal-related complications did not occur.
Conclusion
Peripheral TIVAP placement in the forearms of children is a feasible, effective and safe technique with good midterm outcome. As results are comparable with standard access routes, this technique may be offered as an alternative when intermittent venous access is required.
