Abstract
Fabry disease (FD) is a life-limiting disorder characterized by intracellular globotriaosylceramide (Gb3) accumulations. The underlying α-galactosidase A (α-GAL A) deficiency is caused by variants in the gene GLA. Variants of unknown significance (VUS) are frequently found in GLA and challenge clinical management. Here, we investigated a 49-year old man with cryptogenic lacunar cerebral stroke and the chance finding of the VUS S126G, who was sent to our center for diagnosis and initiation of a costly and life-long FD-specific treatment. We combined clinical examination with in vitro investigations of dermal fibroblasts (HDF), induced pluripotent stem cells (iPSC), and iPSC-derived sensory neurons. We analyzed α-GAL A activity in iPSC, Gb3 accumulation in all three cell types, and action potential firing in sensory neurons. Neurological examination and small nerve fiber assessment was normal except for reduced distal skin innervation. S126G iPSC showed normal α-GAL A activity compared to controls and no Gb3 deposits were found in all three cell types. Baseline electrophysiological characteristics of S126G neurons showed no difference compared to healthy controls as investigated by patch-clamp recordings. We pioneer multi-level cellular characterization of the VUS S126G using three cell types derived from a patient and provide further evidence for the benign nature of S126G in GLA, which is of great importance in the management of such cases in clinical practice.
Affective and cognitive behavior in the alpha-galactosidase A deficient mouse model of Fabry disease

Fabry disease is an X-linked inherited lysosomal storage disorder with intracellular accumulation of globotriaosylceramide (Gb3) due to α-galactosidase A (α-Gal A) deficiency. Fabry patients frequently report of anxiety, depression, and impaired cognitive function. We characterized affective and cognitive phenotype of male mice with α-Gal A deficiency (Fabry KO) and compared results with those of age-matched male wildtype (WT) littermates. Young (3 months) and old (≥ 18 months) mice were tested in the naïve state and after i.pl. injection of complete Freund`s adjuvant (CFA) as an inflammatory pain model. We used the elevated plus maze (EPM), the light-dark box (LDB) and the open field test (OF) to investigate anxiety-like behavior. The forced swim test (FST) and Morris water maze (MWM) were applied to assess depressive-like and learning behavior. The EPM test revealed no intergroup difference for anxiety-like behavior in naïve young and old Fabry KO mice compared to WT littermates, except for longer time spent in open arms of the EPM for young WT mice compared to young Fabry KO mice (p<0.05). After CFA injection, young Fabry KO mice showed increased anxiety-like behavior compared to young WT littermates (p<0.05) and naïve young Fabry KO mice (p<0.05) in the EPM as reflected by shorter time spent in EPM open arms. There were no relevant differences in the LDB and the OF test, except for longer time spent in the center zone of the OF by young WT mice compared to young Fabry KO mice (p<0.05). Complementary to this, depression-like and learning behavior were not different between genotypes and age-groups, except for the expectedly lower memory performance in older age-groups compared to young mice. Our results indicate that genetic influences on affective and cognitive symptoms in FD may be of subordinate relevance, drawing attention to potential influences of environmental and epigenetic factors.
Background
Fabry disease (FD) is an X-linked multisystemic disorder with a heterogeneous phenotype. Especially atypical or late-onset type 2 phenotypes present a therapeutical dilemma.
Methods
To determine the clinical impact of the alpha-Galactosidase A (GLA) p.A143T/ c.427G > A variation, we retrospectively analyzed 25 p.A143T patients in comparison to 58 FD patients with other missense mutations.
Results
p.A143T patients suffering from stroke/ transient ischemic attacks had slightly decreased residual GLA activities, and/or increased lyso-Gb3 levels, suspecting FD. However, most male p.A143T patients presented with significant residual GLA activity (~50 % of reference), which was associated with normal lyso-Gb3 levels. Additionally, p.A143T patients showed less severe FD-typical symptoms and absent FD-typical renal and cardiac involvement in comparison to FD patients with other missense mutations. Two tested female p.A143T patients with stroke/TIA did not show skewed X chromosome inactivation. No accumulation of neurologic events in family members of p.A143T patients with stroke/transient ischemic attacks was observed.
Conclusions
We conclude that GLA p.A143T seems to be most likely a neutral variant or a possible modifier instead of a disease-causing mutation. Therefore, we suggest that p.A143T patients with stroke/transient ischemic attacks of unknown etiology should be further evaluated, since the diagnosis of FD is not probable and subsequent ERT or chaperone treatment should not be an unreflected option.
Background
Fabry disease (FD) is an X-linked recessive hereditary lysosomal storage disorder which results in the accumulation of globotriaosylceramid (Gb3) in tissues of kidney and heart as well as central and peripheral nervous system.
Besides prominent renal and cardiac organ involvement, cochlear symptoms like high-frequency hearing loss and tinnitus are frequently found with yet no comprehensive data available in the literature.
Objective
To examine hearing loss in patients with FD depending on cardiac and renal function.
Material and methods
Single-center study with 68 FD patients enrolled between 2012 and 2016 at the Department of Oto-Rhino-Laryngology, Plastic, Aesthetic and Reconstructive Head and Neck Surgery of the University of Würzburg. Every subject underwent an oto-rhino-laryngological examination as well as behavioral, electrophysiological and electroacoustical audiological testing. High-frequency thresholds were evaluated by using a modified PTA\(_{6}\) (0.5, 1, 2, 4, 6, 8) and HF-PTA (6, 8 kHz). Renal function was measured by eGFR, cardiac impairment was graduated by NYHA class.
Results
Sensorineural hearing loss was detected in 58.8% of the cohort, which occurred typically in sudden episodes and affected especially high frequencies. Hearing loss is asymmetric, beginning unilaterally and affecting the contralateral ear later. Tinnitus was reported by 41.2%. Renal and cardiac impairment influenced the severity of hearing loss (p < 0.05).
Conclusions
High frequency hearing loss is a common problem in patients with FD. Although not life-threatening, it can seriously reduce quality of life and should be taken into account in diagnosis and therapy. Optimized extensive hearing assessment including higher frequency thresholds should be used.
Long-term treatment effect studies in large female Fabry patient groups are challenging to design because of phenotype heterogeneity and lack of appropriate comparison groups, and have not been reported. We compared long-term cardiomyopathy and kidney function outcomes after agalsidase beta treatment with preceding treatment-naive outcomes.
Methods and results Self-controlled pretreatment and post-treatment comparison (piecewise mixed linear modelling) included Fabry female patients ≥18 years at treatment initiation who received agalsidase beta (0.9–1.1 mg/kg every other week) for ≥2 years, with ≥2 pretreatment and ≥2 post-treatment outcome measurements during 10-year follow-up. Left ventricular posterior wall thickness (LVPWT)/interventricular septal thickness (IVST) and estimated glomerular filtration rate (eGFR, Chronic Kidney Disease Epidemiology Collaboration creatinine equation) analyses included 42 and 86 patients, respectively, aged 50.0 and 46.3 years at treatment initiation, respectively. LVPWT and IVST increased pretreatment (follow-up 3.5 years) but stabilized during 3.6 years of treatment (LVPWT: n = 38, slope difference [95% confidence interval (CI)] = - 0.41 [ - 0.68, - 0.15] mm/year, P\(_{pre–post difference}\)<0.01; IVST: n = 38, slope difference =-0.32 [-0.67, 0.02] mm/year, P\(_{pre–post difference}\) = 0.07). These findings were not modified by renal involvement or antiproteinuric agent use. Compared with the treatment-naive period (follow-up 3.6 years), eGFR decline remained modest and stabilized within normal ranges during 4.1 years of treatment (slope difference, 95% CI: -0.13 [-1.15, 0.89] mL/min/1.73m\(^2\)/year, P\(_{pre–post difference}\) = 0.80).
Conclusions Cardiac hypertrophy, progressing during pretreatment follow-up, appeared to stabilize during sustained agalsidase beta treatment. eGFR decline remained within normal ranges. This suggests that treatment may prevent further Fabry-related progression of cardiomyopathy in female patients and maintain normal kidney function.
Background
X-chromosomal inheritance patterns and generally rare occurrence of Fabry disease (FD) account for mono-mutational hemizygous male and heterozygous female patients. Female mutation carriers are usually clinically much less severely affected, which has been explained by a suggested mosaicism in cell phenotype due to random allele shutdown. However, clinical evidence is scarce and potential additional effects in female gene carriers, which might account for specific clinical characteristics such as less severe chronic kidney disease, are yet unknown.
Case presentation
This article reports on a 45 year old female patient carrying the two alpha-galactosidase A gene mutations c.416A > G, p.N139S in exon 3 and c.708G > C, p.W236C in exon 5, but still showing only mild organ manifestations.
Conclusion
This current case highlights the importance of careful clinical characterization in patients with Fabry disease, who may show additional rare constellations and, therefore, are in need of personalized medicine. The impact of potential additional protective effects exceeding the presence of a non-pathogenic GLA allele in female gene carriers requires further investigation.
Background
Fabry Disease (FD) is an X-linked hereditary lysosomal storage disorder which leads to a multisystemic intralysosomal accumulation of globotriaosylceramid (Gb3). Besides prominent renal and cardiac organ involvement, patients commonly complain about vestibulocochlear symptoms like high-frequency hearing loss, tinnitus and vertigo. However, comprehensive data especially on vertigo remain scarce. The aim of this study was to examine the prevalence and characteristics of vertigo and hearing loss in patients with FD, depending on renal and cardiac parameters and get hints about the site and the pattern of the lesions.
Methods
Single-center study with 57 FD patients. Every patient underwent an oto-rhino-laryngological examination as well as videonystagmography and vestibular evoked myogenic potentials (VEMPs) and audiological measurements using pure tone audiometry and auditory brainstem response audiometry (ABR). Renal function was measured by eGFR, cardiac impairment was graduated by NYHA class.
Results
More than one out of three patients (35.1%) complained about hearing loss, 54.4% about vertigo and 28.1% about both symptom. In 74% a sensorineural hearing loss of at least 25 dB was found, ABR could exclude any retrocochlear lesion. Caloric testing showed abnormal values in 71.9%, VEMPs were pathological in 68%. A correlation between the side or the shape of hearing loss and pathological vestibular testing could not be revealed.
Conclusions
Hearing loss and vertigo show a high prevalence in FD. While hearing loss seems due to a cochlear lesion, peripheral vestibular as well as central nervous pathologies cause vertigo. Thus, both the site of lesion and the pathophysiological patterns seem to differ.
Fabry disease is an X-linked lysosomal storage disorder due to impaired activity of alpha-galactosidase A with intracellular accumulation of globotriaosylceramide. Associated small fiber pathology leads to characteristic pain in Fabry disease. We systematically assessed sensory system, physical activity, metabolic parameters, and morphology of male and female mice with alpha-galactosidase A deficiency (Fabry ko) from 2 to 27 months of age and compared results with those of age- and gender-matched wild-type littermates of C57Bl/6J background. Results From the age of two months, male and female Fabry mice showed mechanical hypersensitivity (p < 0.001 each) compared to wild-type littermates. Young Fabry ko mice of both genders were hypersensitive to heat stimulation (p < 0.01) and developed heat hyposensitivity with aging (p < 0.05), while cold hyposensitivity was present constantly in young (p < 0.01) and old (p < 0.05) Fabry ko mice compared to wild-type littermates. Stride angle increased only in male Fabry ko mice with aging (p < 0.01) in comparison to wild-type littermates. Except for young female mice, male (p < 0.05) and female (p < 0.01) Fabry ko mice had a higher body weight than wild-type littermates. Old male Fabry ko mice were physically less active than their wild-type littermates (p < 0.05), had lower chow intake (p < 0.001), and lost more weight (p < 0.001) in a one-week treadmill experiment than wild-type littermates. Also, Fabry ko mice showed spontaneous pain protective behavior and developed orofacial dysmorphism resembling patients with Fabry disease.
Conclusions. Mice with alpha-galactosidase A deficiency show age-dependent and distinct deficits of the sensory system. alpha-galactosidase A-deficient mice seem to model human Fabry disease and may be helpful when studying the pathophysiology of Fabry-associated pain.
Der Morbus Fabry ist eine X-chromosomal rezessive lysosomale Speicherkrankheit, es resultiert eine verminderte Aktivität des Enzyms alpha-Galaktosidase-A. Diese führt zu einer Einlagerung von Globotriaosylceramiden in verschiedenen Organsystemen. Neben Niere und Nervensystem ist das Herz einer der Hauptmanifestationsorte der Erkrankung. Der Morbus Fabry führt unbehandelt zu einer ventrikulären Hypertrophie, verminderten linksventrikulären Funktion und schließlich zu einer myokardialen Fibrosierung. Viele Patienten sterben aufgrund einer progredienten Herzinsuffizienz. Seit 2001 steht mit der Enzymersatztherapie (ERT), die alpha-Galaktosidase substituiert, eine kausale Behandlung des Enzymdefekts zur Verfügung. Erste, auf einen kurzen Zeitraum (bis zu 12 Monate) angelegte, klinische Studien bei Patienten mit Morbus Fabry haben positive Effekte in Hinblick auf die Funktion und Morphologie des Herzens bei Fabry-Patienten gezeigt. Jedoch zeigten die untersuchten Patienten untereinander oft deutlich unterschiedliche Therapieeffekte. Die Langzeiteffekte einer Enzymersatztherapie, insbesondere in Hinblick auf eine zunehmende Fibrosierung des Herzens als Prognose-Parameter im Laufe der Erkrankung, wurden bisher nicht untersucht. Auch fehlen Daten für eine Aussage über den frühestnötigen Therapiezeitpunkt. Diese Untersuchungen erfolgen zum ersten Mal im Rahmen dieser Studie. Es wurden 30 Patienten (42±7 Jahre) mit genetisch gesichertem Morbus Fabry vor Therapie und nach 1, 2 und 3 Jahren unter Enzymersatztherapie untersucht. Behandelt wurde mit 1.0 mg/kg Körpergewicht rekombinanter alpha-Galaktosidase A (agalsidase ß, Fabrazyme®). Es erfolgten Magnetresonanztomographie- und echokardiographische Untersuchungen. Die echokardiographischen Untersuchungsergebnisse wurden mit einer Kohorte von 20 Herzgesunden verglichen. Neben der Bestimmung echokardiographischer Standardwerte wie der Septum- und Hinterwandstärke und der diastolischen Funktion erfolgte eine Evaluierung der regionalen myokardialen Funktion mittels Gewebedoppler (Strain und Strain Rate Imaging sowie Double Peak-Technik). Im Magnetresonanztomographen (MRT) erfolgte die Detektion eines eventuellen Late Enhancements als Marker für myokardiale Fibrose. Die Patienten wurden anhand des Late Enhancements im MRT in drei Gruppen eingeteilt: Keine Fibrose (n=12), Fibrose in einer (n=9) und Fibrose in mehreren Regionen (n=9). Nur die Gruppe, die Baseline keine Fibrose aufwies zeigte unter dreijähriger ERT eine Normalisierung der Wanddicke und eine funktionelle Normalisierung der regionalen Herzfunktion (Strain Rate radial: von 2,3±0,4s-1 auf 2,9±0,7s-1; p<0,05; Vergleichskollektiv: 2,8±0,5s-1). Die anderen beiden Gruppen zeigten zwar einen Rückgang der Hypertrophie, hinsichtlich der Herzfunktion konnten sie jedoch bei bereits deutlich erniedrigten Funktionswerten zum Baseline-Zeitpunkt lediglich stabilisiert werden. Bei rechtzeitigem Therapiebeginn scheint die Enzymersatztherapie eine effektive Behandlung des Herzens bei Morbus Fabry zu ermöglichen. Diese Langzeitstudie zur Enzymersatztherapie bei Morbus Fabry über 3 Jahre zeigt jedoch deutlich, dass dies nur bei noch nicht fibrotisch verändertem Herzen gilt. Die Indikation zur Enzymersatztherapie sollte daher aus kardiologischer Sicht frühzeitig gestellt werden.
Einführung M. Fabry ist eine x-chromosomal vererbte Enzymmangelerkrankung und führt zu progressiver Niereninsuffizienz und hypertropher Kardiomyopathie. Ziel dieser Studie war die Analyse der kardialen Veränderungen im Rahmen der Erkrankung und unter Enzymersatztherapie (ERT) mittels kontrastmittelgestützter Magnetresonanztomographie (MRT). Material und Methoden 25 Patienten (4 Frauen, Alter 18-55 Jahre) mit genetisch bestätigtem M. Fabry wurden vor und nach 12 Monaten ERT mit Fabrazyme® untersucht. Es erfolgte eine umfassende Analyse morphologischer und funktioneller Parameter des linken Ventrikels (LV) mit Fokussierung auf regionale Wandveränderungen sowie Late Enhancement (LE). Eine Gruppe von 43 gesunden Probanden diente als Kontrollgruppe. Ergebnisse Vor Therpiebeginn war bei 50% aller Patienten eine LV Hypertrophie mit erhöhter enddiastolischen Wanddicke (EDWT) nachweisbar. Die ERT führte zu einer Reduktion der EDWT und der LV Masse um 3,6 bis 10,3% innerhalb von 12 Monaten. Die initial erhöhten LV Volumina nahmen ebenfalls unter Therapie ab. Männliche Patienten wiesen eine eingeschränkte Ejektionsfraktion (EF) auf als Zeichen einer globalen Herzinsuffizienz, welche sich unter Therapie signifikant verbesserte. Die segmentale Analyse der systolischen Wanddickenzunahme (WT) zeigte teils ausgedehnte Wandbewegungsstörungen und eine Verbesserung der Kontraktilität unter ERT. Letztere war am stärksten ausgeprägt, wenn kein LE vorhanden war. Ein LE in der LV Seitenwand ist charakteristisch für M. Fabry und konnte bei 11 von 21 männlichen Patienten nachgewiesen werden mit einem Volumen von durchschnittlich 1,9 ± 1,8% der LV Masse. Das Ausmaß nahm häufig unter Therapie noch zu, es kam zu keiner Reduktion. Allerdings wurde auch kein Neuauftreten beobachtet. Frauen waren nicht betroffen. Das Alter LE-positiver Patienten lag etwa 10 Jahre über dem LE-negativer Patienten. Im Frauenkollektiv war die LV Hypertrophie geringer ausgeprägt als bei männlichen Patienten, lag aber dennoch über dem Normwert. Zwar war der Schweregrad der segmentalen Hypertrophie geringer, aber die regionale Verteilung war dennoch ähnlich der bei männlichen Patienten und blieb unter ERT auf hohem Niveau konstant. Auch die regionale Kontraktilität war mäßig beeinträchtigt und durchaus vergleichbar mit dem Ausmaß der Wandbewegungsstörungen bei LE-negativen männlichen Patienten. Bei Frauen war keine globale LV Insuffizienz nachweisbar. Dennoch belegen die erhöhte LV Wanddicke und segmentale Hypokinesien eine messbare kardiale Beeinträchtigung durch M. Fabry auch bei heterozygoten Patienten. Fazit Insgesamt erwies sich die MRT des Herzens als ideales Instrument zur strahlungsfreien Überwachung von Fabry Patienten unter ERT. So können bereits diskrete und regional beschränkte Veränderungen mit hoher Reliabilität diagnostiziert werden, auch in Regionen, die der Echokardiographie nur schwer zugänglich sind. Die LV Masse ist bereits in frühen Krankheitsstadien erhöht und korreliert gut mit der allgemeinen Erkrankungsschwere. Desweiteren impliziert die enge Verknüpfung zwischen Hypertrophie, erhöhten LV Volumina, Wandbewegungsstörungen und letztenendes globaler Herzinsuffizienz, dass die LV Hypertrophie einen wichtigen und früh sichtbaren Risikofaktor darstellt. Das LE als Zeichen der myokardialen Fibrose in späten Erkrankungsstadien ist irreversibel, ebenfalls eng verbunden mit der kardialen Funktion und scheint ein weiterer Risikofaktor zu sein für ein vermindertes Therapieansprechen. Mit dem Auftreten eines LE und zunehmenden LE-Scores erhöht sich die Wahrscheinlichkeit des Auftretens einer LV Hypertrophie und von Wandbewegungsstörungen. Das Patientenalter hatte keinen offensichtlichen Einfluss auf die Wirkung der ERT. Es gibt jedoch Hinweise auf eine Art „point of no return“, jenseits dessen der Therapieeffekt - zumindest das Herz betreffend - begrenzt zu sein scheint.
Fabry disease (FD) is a rare life-threatening disorder caused by deficiency of the alpha-galactosidase A (GLA) enzyme with a characteristic pain phenotype. Impaired GLA production or function leads to the accumulation of the cell membrane compound globotriaosylceramide (Gb3) in the neurons of the dorsal root ganglia (DRG) of FD patients. Applying immunohistochemistry (IHC) and quantitative real-time polymerase chain reaction (qRT PCR) analysis on DRG tissue of the GLA knockout (KO) mouse model of FD, we address the question of how Gb3 accumulation may contribute to FD pain and focus on the immune system and pain-associated ion channel gene expression. We show a higher Gb3 load in the DRG of young (<6 months) (p < 0.01) and old (≥12 months) (p < 0.001) GLA KO mice compared to old wildtype (WT) littermates, and an overall suppressed immune response in the DRG of old GLA KO mice, represented by a reduced number of CD206\(^+\) macrophages (p < 0.01) and lower gene expression levels of the inflammation-associated targets interleukin(IL)1b (p < 0.05), IL10 (p < 0.001), glial fibrillary acidic protein (GFAP) (p < 0.05), and leucine rich alpha-2-glycoprotein 1 (LRG1) (p < 0.01) in the DRG of old GLA KO mice compared to old WT. Dysregulation of immune-related genes may be linked to lower gene expression levels of the pain-associated ion channels calcium-activated potassium channel 3.1 (KCa3.1) and transient receptor potential ankyrin 1 channel (TRPA1). Ion channel expression might further be disturbed by impaired sphingolipid recruitment mediated via the lipid raft marker flotillin-1 (FLOT1). This impairment is represented by an increased number of FLOT1\(^+\) DRG neurons with a membranous expression pattern in old GLA KO mice compared to young GLA KO, young WT, and old WT mice (p < 0.001 each). Further, we provide evidence for aberrant behavior of GLA KO mice, which might be linked to dysregulated ion channel gene expression levels and disturbed FLOT1 distribution patterns. Behavioral testing revealed mechanical hypersensitivity in young (p < 0.01) and old (p < 0.001) GLA KO mice compared to WT, heat hypersensitivity in young GLA KO mice (p < 0.001) compared to WT, age-dependent heat hyposensitivity in old GLA KO mice (p < 0.001) compared to young GLA KO mice, and cold hyposensitivity in young (p < 0.001) and old (p < 0.001) GLA KO mice compared to WT, which well reflects the clinical phenotype observed in FD patients.
Bei Morbus Fabry handelt es sich um eine X-chromosomal rezessiv vererbbare lysosomale Speichererkrankung. Im Vordergrund der kardialen Beteiligung stehen eine progrediente Herzinsuffizienz, bedingt durch eine linksventrikuläre Hypertrophie mit kardialer Fibrosierung, sowie eine Mitbeteiligung des Reizleitungssystems.
Bei 150 Patienten wurden im Zeitraum von 2001-2009 neben einer klinischen Untersuchung ein EKG, eine Echokardiographie, ein Belastungs-EKG und teilweise auch eine Magnetresonanztomographie durchgeführt. Zum Vergleich der Patientenentwicklung wurde jeweils das jüngste Follow-up Ergebnis mit den Baseline-Daten verglichen.
Es konnte eine signifikante Korrelation zwischen der QRS-Dauer und der Wandstärke in der Echokardiographie und der Magnetresonanztomographie eindeutig nachgewiesen werden. Eine myokardiale Fibrose ist bei normalen Ruhe-EKG-Parametern nahezu auszuschließen. In der Untersuchung des Langzeit-EKGs fanden sich bei einigen Patienten höhergradige ventrikuläre Rhythmusstörungen, welche als erhöhtes individuelles Risiko und als bedeutender Faktor der Sterblichkeit bei Morbus Fabry zu werten sind.
Background
Pain is an early symptom of Fabry disease (FD) and is characterized by a unique phenotype with mainly episodic acral and triggerable burning pain. Recently, we designed and validated the first pain questionnaire for adult FD patients in an interview and a self-administered version in German: the Wurzburg Fabry Pain Questionnaire (FPQ). We now report the validation of the English version of the self-administered FPQ (enFPQ).
Methods
After two forward-backward translations of the FPQ by native German and native English speakers, the enFPQ was applied at The Mark Holland Metabolic Unit, Manchester, UK for validation. Consecutive patients with genetically ascertained FD and current or previous FD pain underwent a face-to-face interview using the enFPQ. Two weeks later, patients filled in the self-administered enFPQ at home. The agreement between entries collected by supervised administration and self-administration of the enFPQ was assessed via Gwet's AC1-statistics (AC1) for nominal-scaled scores and intraclass correlation coefficient (ICC) for interval-scaled elements.
Results
Eighty-three FD patients underwent the face-to-face interview and 54 patients sent back a completed self-administered version of the enFPQ 2 weeks later. We found high agreement with a mean AC1-statistics of 0.725 for 55 items, and very high agreement with a mean ICC of 0.811 for 9 items.
Conclusions
We provide the validated English version of the FPQ for self-administration in adult FD patients. The enFPQ collects detailed information on the individual FD pain phenotype and thus builds a solid basis for better pain classification and treatment in patients with FD.
Background
Anderson–Fabry disease (FD) is an X-linked lysosomal storage disorder with varying organ involvement and symptoms, depending on the underlying mutation in the alpha-galactosidase A gene (HGNC: GLA). With genetic testing becoming more readily available, it is crucial to precisely evaluate pathogenicity of each genetic variant, in order to determine whether there is or might be not a need for FD-specific therapy in affected patients and relatives at the time point of presentation or in the future.
Methods
This case series investigates the clinical impact of the specific GLA gene variant c.376A>G (p.Ser126Gly) in five (one heterozygous and one homozygous female, three males) individuals from different families, who visited our center between 2009 and 2021. Comprehensive neurological, nephrological and cardiac examinations were performed in all cases. One patient received a follow-up examination after 12 years.
Results
Index events leading to suspicion of FD were mainly unspecific neurological symptoms. However, FD-specific biomarkers, imaging examinations (i.e., brain MRI, heart MRI), and tissue-specific diagnostics, including kidney and skin biopsies, did not reveal evidence for FD-specific symptoms or organ involvement but showed normal results in all cases. This includes findings from 12-year follow-up in one patient with renal biopsy.
Conclusion
These findings suggest that p.Ser126Gly represents a benign GLA gene variant which per se does not cause FD. Precise clinical evaluation in individuals diagnosed with genetic variations of unknown significance should be performed to distinguish common symptoms broadly prevalent in the general population from those secondary to FD.
Background
Fabry disease is an inborn lysosomal storage disorder which is associated with small fiber neuropathy. We set out to investigate small fiber conduction in Fabry patients using pain-related evoked potentials (PREP).
Methods
In this case–control study we prospectively studied 76 consecutive Fabry patients for electrical small fiber conduction in correlation with small fiber function and morphology. Data were compared with healthy controls using non-parametric statistical tests. All patients underwent neurological examination and were investigated with pain and depression questionnaires. Small fiber function (quantitative sensory testing, QST), morphology (skin punch biopsy), and electrical conduction (PREP) were assessed and correlated. Patients were stratified for gender and disease severity as reflected by renal function.
Results
All Fabry patients (31 men, 45 women) had small fiber neuropathy. Men with Fabry disease showed impaired cold (p < 0.01) and warm perception (p < 0.05), while women did not differ from controls. Intraepidermal nerve fiber density (IENFD) was reduced at the lower leg (p < 0.001) and the back (p < 0.05) mainly of men with impaired renal function. When investigating A-delta fiber conduction with PREP, men but not women with Fabry disease had lower amplitudes upon stimulation at face (p < 0.01), hands (p < 0.05), and feet (p < 0.01) compared to controls. PREP amplitudes further decreased with advance in disease severity. PREP amplitudes and warm (p < 0.05) and cold detection thresholds (p < 0.01) at the feet correlated positively in male patients.
Conclusion
Small fiber conduction is impaired in men with Fabry disease and worsens with advanced disease severity. PREP are well-suited to measure A-delta fiber conduction.
Der Morbus Fabry ist eine lysosomale Speicherkrankheit, die auf einem Mangel des Enzyms a-Galaktosidase A beruht. Die Krankheit wird X-chromosomal rezessiv vererbt und entsteht durch Mutation des a-Galaktosidase-Gens auf dem langen Arm des Chromosoms Xq22. Durch die erniedrigte bzw. fehlende Enzymaktivität kommt es zu einer übermäßigen Ablagerung von Glykosphingolipiden in sämtlichen Geweben des menschlichen Körpers, besonders betroffen sind Herz, Nieren, Gefäße und ZNS. Die Krankheit ist durch einen progredienten Verlauf und einer eingeschränkten Lebenserwartung gekennzeichnet. Insbesondere die kardialen Auswirkungen wie Herzrhythmusstörungen, Klappenvitien und linksventrikuläre Hypertrophie führen zur Herzinsuffizienz und fast immer zu einem meist frühzeitigen Tod durch Herzversagen. Seit einiger Zeit steht in der Enzymersatztherapie mit rekombinanter a-Galaktosidase A (Agalsidase) eine kausale Behandlung zur Verfügung. Unter der Therapie mit Agalsidase zeigen sich auch Verbesserungen der kardialen Parameter, insbesondere eine Reduktion der linksventrikulären Masse. Zur Kontrolle und zur Dokumentation der medikamentösen Wirkung an den verschiedenen Organen waren und sind klinische Studien und Untersuchungen der betroffenen Patienten notwendig. Zur Beurteilung der kardialen Funktion steht, neben den bekannten Routineverfahren wie der Echokardiographie und der MR-Bildgebung, mit der 31P-Magnetresonanz-Spektroskopie ein nicht invasives Verfahren zur Beurteilung des myokardialen Stoffwechsels zur Verfügung. Mit Hilfe von speziellen Auswerteprogrammen können die Absolutkonzentrationen von energiereichen Metaboliten, besonders von Phosphokreatin und Adenosintriphosphat, im Herzmuskel in vivo bestimmt werden. Ziel der vorliegenden Arbeit war zunächst einmal die Messung der Konzentrationen der energiereichen Metabolite im Myokard von Patienten mit Morbus Fabry und der Vergleich der Daten mit denen von gesunden Probanden. Des weiteren wurde die Patientengruppe unter Therapie mit Agalsidase b einer frühen und einer späten Kontrolluntersuchung mittels MR-Spektroskopie unterzogen, um Veränderungen im kardialen Metabolismus darzustellen. Die spektroskopischen Daten gaben Aufschluss über Ausmaß der Beeinträchtigung des myokardialen Stoffwechsels aufgrund der Gb3-Ablagerungen und ergänzten die klinischen und bildmorphologischen Untersuchungen. Hierbei konnte eine tendenzieller Anstieg der PCr- und ATP-Konzentrationen unter ERT im Myokard nachgewiesen werden, gleichfalls zeigten sich in dem untersuchten Kollektiv eine Abnahme der linksventrikulären Masse und eine erhöhte Ejektionsfraktion. Ebenso konnte dargelegt werden, dass wie auch bei anderen Herzerkrankungen, wie zum Beispiel der dilatativen Kardiomyopathie oder der koronaren Herzkrankheit, bei einer Stoffwechselerkrankung wie der Fabry-Krankheit deutlich verringerte Konzentrationen energiereicher Phosphate in den Herzmuskelzellen vorliegen.
Background: Impairments of health related quality of life (HRQoL) are frequently observed in Fabry disease (FD) and are known to be related to neuropathic pain and cardiovascular events. This study aimed to explore the role of chronic kidney disease (CKD) in a large cohort of patients with FD.
Methods: In 96 patients (53% female; age 40 ± 12 yrs) with genetically proven FD, HRQoL was assessed by the Medical Outcomes Study (SF-36) questionnaire. All patients were naïve to enzyme replacement therapy. Three categories for kidney dysfunction were chosen, eGFR ≥/<60 ml/min/1.73 m2 or need of renal replacement therapy (RRT). Minor (e.g. arrhythmia, angina pectoris, etc.) and major (e.g. myocardial infarction, coronary artery bypass, stroke or implantable cardioverter-defibrillator) vascular events as well as pain and pain therapy were considered in linear regression analyses with the dimensions of HRQoL.
Results: Ten patients (10%) had impaired kidney function and a further nine were on RRT (9.4%). Kidney function and pain emerged as the main factors associated with lower scores on the SF 36, in particular on physical components (PCS beta-coefficients for CKD −6.2, for RRT −11.8, for pain −9.1, p < 0.05, respectively), while controlling for gender, vascular event and pain-therapy. Relationships were found for mental aspects of HRQoL. Age and history of vascular events were not related to HRQoL.
Conclusion: Cardiovascular events and pain are important factors related to HRQoL, social functioning and depression. Our study highlights impaired chronic kidney disease, in particular after initiation of RRT, as a strong determinant of reduced HRQoL in FD.
The long-term effects of enzyme-replacement therapy (ERT) in Fabry disease are unknown. Thus, the aim of this study was to determine whether ERT in patients with advanced Fabry disease affects progression towards 'hard' clinical end-points in comparison with the natural course of the disease.
METHODS:
A total of 40 patients with genetically proven Fabry disease (mean age 40 ± 9 years; n = 9 women) were treated prospectively with ERT for 6 years. In addition, 40 subjects from the Fabry Registry, matched for age, sex, chronic kidney disease stage and previous transient ischaemic attack (TIA), served as a comparison group. The main outcome was a composite of stroke, end-stage renal disease (ESRD) and death. Secondary outcomes included changes in myocardial left ventricular (LV) wall thickness and replacement fibrosis, change in glomerular filtration rate (GFR), new TIA and change in neuropathic pain.
RESULTS:
During a median follow-up of 6.0 years (bottom and top quartiles: 5.1, 7.2), 15 events occurred in 13 patients (n = 7 deaths, n = 4 cases of ESRD and n = 4 strokes). Sudden death occurred (n = 6) only in patients with documented ventricular tachycardia and myocardial replacement fibrosis. The annual progression of myocardial LV fibrosis in the entire cohort was 0.6 ± 0.7%. As a result, posterior end-diastolic wall thinning was observed (baseline, 13.2 ± 2.0 mm; follow-up, 11.4 ± 2.1 mm; P < 0.01). GFR decreased by 2.3 ± 4.6 mL min(-1) per year. Three patients experienced a TIA. The major clinical symptom was neuropathic pain (n = 37), and this symptom improved in 25 patients. The event rate was not different between the ERT group and the untreated (natural history) group of the Fabry Registry.
CONCLUSION:
Despite ERT, clinically meaningful events including sudden cardiac death continue to develop in patients with advanced Fabry disease.
Background
The aim of the present study was to assess manifestations of and applied treatment concepts for females with Fabry disease (FD) according to the current European Fabry Guidelines.
Methods
Between 10/2008 and 12/2014, data from the most recent visit of 261 adult female FD patients from six German Fabry centers were retrospectively analyzed. Clinical presentation and laboratory data, including plasma lyso-Gb3 levels were assessed.
Results
Fifty-five percent of females were on enzyme replacement therapy (ERT), according to recent European FD guidelines. Thirty-three percent of females were untreated although criteria for ERT initiation were fulfilled. In general, the presence of left ventricular hypertrophy (LVH) seemed to impact more on ERT initiation than impaired renal function. In ERT-naïve females RAAS blockers were more often prescribed if LVH was present rather than albuminuria. Affected females with missense mutations showed a similar disease burden compared to females with nonsense mutations. Elevated plasma lyso-Gb3 levels in ERT-naïve females seem to be a marker of disease burden, since patients showed comparable incidences of organ manifestations even if they were ~8 years younger than females with normal lyso-Gb3 levels.
Conclusion
The treatment of the majority of females with FD in Germany is in line with the current European FD guidelines. However, a relevant number of females remain untreated despite organ involvement, necessitating a careful reevaluation of these females.
