This paper deals with the importance of music lessons in Bavarian special needs schools. It is based on three hypotheses and develops various possible reasons regarding the above-mentioned concern in relation to the focus on schools working with students' challenging behaviour.
For this purpose, literature and an expert interview are used to derive special conditions of these schools. Moreover, this paper examines the circumstances of musical education and possible ways of dealing with them at schools.
The aim of this paper is to derive general possibilities for teaching. The concept of 'music education units' (Musikvermittlungseinheiten) is used as a basis for suggesting a concrete way of integrating music education into everyday school life. These are short, approximately 10-minute units in which music is used and taught as part of structuring and rhythmising phases. Chances and limits of these music education units as well as recommendations for action and exemplary suggestions for their design are elaborated at the end of the paper.
Das Projekt wertet zwei Fernsehserien der 1970er Jahre aus, deren Erhalt bisher unbekannt war: „Patienten“ (1972/73) und „Der schwarze Doktor“ (1975). Das ZDF thematisierte in diesen Produktionen Missstände im Gesundheitswesen wie den stationären sowie ambulanten Ärzt/innenmangel, den Pflegenotstand und die mangelhafte Ärzt/innen-Patient/innen-Beziehung. In der Arbeit werden diese heute noch sehr aktuellen Themen historisch kontextualisiert und die Perspektive der 1970er Jahre erarbeitet.

„Black esophagus“ oder „akute Ösophagusnekrose“ (AÖN) ist eine seltene Erkrankung, die sich makroskopisch durch eine zirkumferente Schwarzverfärbung der Ösophagusmukosa mit abruptem Ende am gastroösophagealen Übergang auszeichnet. Die genaue Pathogenese ist unbekannt; es werden multifaktorielle Einflüsse wie z. B. Säurereflux, Ischämie und verringerte Schutzmechanismen der Mukosa als mögliche Ursachen diskutiert.
Vorgestellt werden 2 Obduktionsfälle, die typische Befunde einer AÖN aufwiesen. Zusätzlich hatten Fall 1 eine Candida-Infektion und Fall 2 eine Appendizitis, sodass eine infektiöse Genese in beiden Fällen eine Rolle gespielt haben könnte.
“In Other News”: China’s International Media Strategy on Xinjiang — CGTN and New China TV on YouTube

In the Western world China stands accused of severe human rights violations regarding its treatment of the Uyghurs and other predominantly Muslim minorities in its northwestern Xinjiang Uyghur Autonomous Region. This is the first article to systematically analyze the response of China’s international state media to these allegations. By studying the YouTube channels of two leading Chinese state media, China Global Television Network (CGTN) and New China TV (operated by Xinhua News Agency), it presents an indepth understanding of how China’s foreign-facing propaganda works in a crucial case. The quantitative content analysis highlights how China reacted to increasing international (mostly United States) pressure regarding its Xinjiang policies by producing higher volumes of videos and putting out new counternarratives. The qualitative analysis that follows provides in-depth treatment of the most important discourses that Chinese media engage in to salvage the nation’s international image, namely those on development, culture, nature, and terrorism. It finds several ways of countering criticism, ranging from presenting a positive image of China, in line with traditional propaganda guidelines and President Xi Jinping’s assignment to state media to “tell the China story well,” to more innovative approaches. Thus the development narrative becomes more personalized, the discourse on culture supports the “heritagization process” to incorporate minority cultures into a harmonized “Chinese civilization,” representations of nature firmly tie Xinjiang into the discourse of “beautiful China,” the “terror narrative” strategically employs shocking footage in an attempt to gain international “discourse power,” etc. The article provides an up-to-date picture of China’s state media strategy on a highly contentious international issue.
Chronic pain conditions are a major reason for the utilization of the health care system. Inflammatory pain states can persist facilitated by peripheral sensitization of nociceptors. The voltage-gated sodium channel 1.9 (NaV1.9) is an important regulator of neuronal excitability and is involved in inflammation-induced pain hypersensitivity. Recently, oxidized 1-palmitoyl-2-arachidonoyl-sn-glycerol-3-phosphatidylcholine (OxPAPC) was identified as a mediator of acute inflammatory pain and persistent hyperalgesia, suggesting an involvement in proalgesic cascades and peripheral sensitization. Peripheral sensitization implies an increase in neuronal excitability. This thesis aims to characterize spontaneous calcium activity in neuronal compartments as a proxy to investigate neuronal excitability, making use of the computational tool Neural Activity Cubic (NA3). NA3 allows automated calcium activity event detection of signal-close-to-noise calcium activity and evaluation of neuronal activity states. Additionally, the influence of OxPAPC and NaV1.9 on the excitability of murine dorsal root ganglion (DRG) neurons and the effect of OxPAPC on the response of DRG neurons towards other inflammatory mediators (prostaglandin E2, histamine, and bradykinin) is investigated. Using calcium imaging, the presence of spontaneous calcium activity in murine DRG neurons was established. NA3 was used to quantify this spontaneous calcium activity, which revealed decreased activity counts in axons and somata of NaV1.9 knockout (KO) neurons compared to wildtype (WT). Incubation of WT DRG neurons with OxPAPC before calcium imaging did not show altered activity counts compared to controls. OxPAPC incubation also did not modify the response of DRG neurons treated with inflammatory mediators. However, the variance ratio computed by NA3 conclusively allowed to determine neuronal activity states. In conclusion, my findings indicate an important function of NaV1.9 in determining the neuronal excitability of DRG neurons in resting states. OxPAPC exposition does not influence neuronal excitability nor sensitizes neurons for other inflammatory mediators. This evidence reduces the primary mechanism of OxPAPC-induced hyperalgesia to acute effects. Importantly, it was possible to establish an approach for unbiased excitability quantification of DRG neurons by calcium activity event detection and calcium trace variance analysis by NA3. It was possible to show that signal-close-to-noise calcium activity reflects neuronal excitability states.
Die Kombination aus zytoreduktiver Chirurgie und einer anschließenden hyperthermen intraperitonealen Chemoperfusion (HIPEC) stellt sich als vielversprechende Therapiestrategie bei ausgesuchten Patienten mit Peritonealkarzinose, z. B. des kolorektalen Karzinoms, dar. Die intraperitoneale Chemoperfusion kombiniert eine lokale Hochdosis-Chemotherapie mit einer Hyperthermie. Hitzeschockproteinen (HSP) kommt dabei eine bedeutende Rolle zu, da sie infolge von zellulären Stressfaktoren wie Hitze oder Zytostatika-bedingter Chemotoxizität induziert werden. HSPs setzen Reparatur- und Zellschutzmechanismen in Gang und vermindern so in einzelnen überlebenden Tumorzellen möglicherweise den gewünschten Therapieerfolg der HIPEC. Ziel der Arbeit war es, mithilfe eines bereits etablierten in vitro HIPEC-Modells die Auswirkungen der äußeren Stressoren Hyperthermie und Zytostatika auf die Expression von Hitzeschockproteinen (HSP27, HSP70 und HSP90) in drei humanen Kolonkarzinomzelllinien zu untersuchen. Dazu wurden die Zelllinien HT29, SW480 und SW620 jeweils mit und ohne Zytostatika (Mitomycin C, MMC und Oxaliplatin, OXA) für eine Stunde drei verschiedenen Temperaturstufen von 37°C als Normothermie, 41°C und 43°C als Hyperthermie ausgesetzt und nach einer Regenerationszeit von 30 min, 24 h, 48 h und 72 h mit Hilfe von RT-qPCR-Analysen und Western Blots untersucht. Zudem wurden nach gleichem Ablauf Effekte der HIPEC auf die Tumorzellproliferation und Apoptose mittels Proliferationsmarkern Ki-67, PCNA und MTS-Tests sowie dem antiapoptotischen Protein Bcl-xL in in vitro Tumorzellansätzen sowie in ex vivo Patientenproben vor und nach HIPEC analysiert. Sowohl die einstündige Chemotherapie mit Mitomycin C oder Oxaliplatin unter hyperthermen Bedingungen als auch die isolierte Hyperthermiebehandlung führte im Vergleich zu normothermen Kontrollbedingungen bei 37°C zu einer signifikanten Überexpression der untersuchten HSPs in RTq-PCR-Analysenaller drei Kolonkarzinomzelllinien. Interessanterweise wurden vermehrte HSP Genexpressionsmuster noch drei Tage nach Behandlung beobachtet. Eine verstärkte Proteinexpression zeigte sich bestätigend insbesondere für HSP27 und HSP70 unter zytostatischer Behandlung mit MMC oder OXA und führte zu einer bis zu 3-fachen Expressionssteigerung wenn die Zellen hyperthermen Bedingungen ausgesetzt waren. Tumorzellen, die zuvor der hyperthermen Chemotherapie unterzogen wurden, zeigten interessanterweise zudem proliferative anstelle von anti-proliferativen Effekten. In durchgeführten MTS-Tests führte sowohl die Hyperthermie allein als auch die zusätzliche Zytostatikagabe zu einer deutlich erhöhten Zellviabilität im Vergleich zu normothermer Chemotherapie im Modellansatz. Übereinstimmend mit den Ergebnissen der MTS-Tests konnte eine Induktion der Proliferationsmarker PCNA und Ki-67 durch Hyperthermie und Chemotherapie auf Gen- und Proteinebene beobachtet werden. Im Falle von PCNA ließ sich eine verstärkte Proteinexpression in ex vivo Proben von Patienten nach klinisch durchgeführter HIPEC bestätigen. Zusätzliche Untersuchungen des anti-apoptotisch wirkenden Regulatorproteins Bcl-xL in in vitro Tumorzellansätzen sowie in ex vivo Proben von Patienten nach hyperthermer Chemotherapie, zeigten zudem eine deutlich gesteigerte Proteinexpression unter alleiniger Hyperthermie sowie insbesondere in Kombination mit Zytostatika. Durch die Induktion von HSP27, HSP70 und HSP90 infolge von hyperthermem und zytotoxischem Stress werden in überlebenden Zellen nach hyperthermer Chemotherapie, unerwünschte antiapopotische sowie proliferative Effekte im Sinne von Reparatur- und Zellschutzmechanismen induziert und nehmen negativen Einfluss auf den Therapieerfolg der HIPEC. Schlussfolgernd wäre der Einsatz von HSP-Inhibitoren um die beschriebenen, unerwünschten Zellmechanismen zu verhindern, zu überprüfen. Diese bieten eine interessante Möglichkeit die Effizienz der im klinischen Einsatz gängigen Zytostatika zu steigern und somit einen positiven Einfluss auf den Erfolg der Therapie und die Überlebenszeit von Patienten mit Peritonealkarzinose zu nehmen. Weiterführende Studien der eigenen Arbeitsgruppe mit kombinierten HSP70/HSP90-Inhibitoren zeigten bereits eine signifikant reduzierte Zellviabilität in Kolonkarzinomzellen, die zuvor der hyperthermen Chemotherapie unterzogen wurden.

Im Rahmen bauvorgreifender Untersuchungen durch die Stadtarchäologie Straubing unter Leitung von Frau Ildiko Bősze konnten in den Jahren 2018 und 2019 im Südosten von Straubing, im Bereich des dortigen Wasserwerks, Teile einer älterlinienbandkeramischen Siedlung erfasst werden. Neben einigen Gruben und vier Hausgrundrissen kamen im nördlichen Bereich der Fläche die Überreste zweier Grubenöfen zu Tage, die hier ausführlich besprochen werden sollen. Als in den letzten Jahren selten anzutreffende Befundgattung und aufgrund ihrer auffällig unterschiedlichen Konstruktionsweisen mit und ohne Steinpflasterung bilden die beiden Straubinger Grubenöfen zwei interessante Untersuchungsobjekte. Mit der Verbreitung der Grubenöfen vom Rheinland bis zum Donauknie und der zeitlichen Beschränkung praktisch ausschließlich auf die LBK handelt es sich um ein Konstruktionsprinzip, das in der Zeit vor den ersten Ackerbauern und auch im anschließenden Mittelneolithikum nicht genutzt wurde (Pechtl 2008, 72–74). Es erscheint daher erstrebenswert, die Befunde sowie die anzusprechende Keramik aus den beiden Grubenöfen aufzuarbeiten und vorzulegen.
Die Arbeit untersucht die Leistungsrelevanz allgemeiner psychischer Leistungsfaktoren im Kontext der Talentförderung im weiblichen Nachwuchsfußball. Hierbei werden die Zusam- menhänge zwischen drei psychischen Dispositionen sowie einem psychischen Zustand und der aktuellen Spielkompetenz untersucht, wobei folgende Leistungsfaktoren fokussiert werden: die dispositionelle Handlungsorientierung, die dispositionelle Lageorientierung, die grundsätzliche Entscheidungskompetenz sowie das situative Flow-Erleben.
Auf Grundlage der Flow-Theorie und der Theorie der Handlungskontrolle sowie dem aktu- ellen Forschungsstand wird angenommen, dass mit steigendem Flow-Erleben eine Zunahme der aktuellen Spielkompetenz einhergeht und dieser psychische Zustand zudem den Zusam- menhang zwischen den ausgewählten Dispositionen und der aktuellen Spielkompetenz voll- ständig vermittelt. Die beiden Theorien und der aktuelle Forschungsstand lassen für eine aus- geprägte dispositionelle Handlungsorientierung sowie eine hohe Entscheidungskompetenz eine Zunahme und für eine ausgeprägte dispositionelle Lageorientierung eine Abnahme des Flow- Erlebens vermuten. Die Zusammenhänge werden mit und ohne vorheriger Stressinduktion und unter Berücksichtigung theoretisch abgeleiteter Kontrollvariablen untersucht.
Der Arbeit liegt eine quantitative Querschnittsstudie zugrunde, die bei 139 Nachwuchsspie- lerinnen aus acht verschiedenen Vereinen in Deutschland durchgeführt wurde. Die Datenaus- wertung erfolgt mittels multipler linearer Regressionsanalysen und linearer Strukturgleichungs- modelle.
Das Flow-Erleben erweist sich als leistungsförderlicher Zustand und verlässlicher Mediator des Zusammenhangs zwischen beiden dispositionellen Arten der Handlungskontrolle und der Spielkompetenz. Die dispositionelle Handlungsorientierung stellt einen positiven und die dis- positionelle Lageorientierung einen negativen Prädiktor für das Flow-Erleben dar. In Bezug auf die Entscheidungskompetenz können die theoretischen Annahmen nicht bestätigt werden. Auf dieser Grundlage erfolgen Implikationen für die sportpädagogische Praxis und die zukünftige Forschungsarbeit zum Flow-Erleben im weiblichen Nachwuchsfußball.

In dieser Arbeit wurde ein Kollektiv chronisch herzinsuffizienter Patienten aus der niedergelassenen kardiologischen Betreuung in Bayern analysiert und auf die Umsetzung der zum Zeitpunkt der HF-Bavaria Studie gültigen Leitlinien untersucht. Dabei wurde das Patientenkollektiv nach dem Geschlecht und zusätzlich auch nach den neu definierten Herzinsuffizienz-Klassen der aktuell gültigen Leitlinien eingeteilt, um Unterschiede und Gemeinsamkeiten innerhalb dieser Differenzierungen darstellen zu können und einen Vergleich zu den Studien der jüngeren Vergangenheit zu ermöglichen.
Die Patienten der HF-Bavaria Studie waren zu 65,9 % männlich (n = 3569) und zu 34,1 % weiblich (n = 1848). Die Frauen litten häufiger unter HFpEF, waren seit kürzerer Zeit herzinsuffizient und waren in der Vergangenheit seltener zur Therapieintensivierung oder Intervention hospitalisiert. Die Patientinnen berichteten dabei weniger häufig Komorbiditäten. So fanden sich bei den Frauen seltener KHK, Niereninsuffizienz oder Diabetes mellitus, hingegen häufiger Herzklappenerkrankungen und Vorhofflimmern. Weiterhin wurden die Patientinnen weniger häufig mit ACE-Hemmer, Betablocker und MRA, dagegen häufiger mit ARB und Digitalis behandelt.
Im Patientenkollektiv der HF-Bavaria Studie hatten 29,0 % eine HFrEF (n = 1581), 28,9 % eine HFmrEF (n = 1577) und 42,0 % eine HFpEF (n = 2291). Patienten mit HFrEF waren überwiegend männlich, zum größten Teil seit mehr als 5 Jahren herzinsuffizient und im Vergleich zu den anderen Herzinsuffizienz-Klassen häufiger in den NYHA-Stadien III und IV eingestuft. HFrEF Patienten hatten den größten Anteil an bereits erfolgten Interventionen und Device-Therapien und die durchschnittlich höchste Anzahl an Komorbiditäten. Das Komorbiditätenspektrum bei Patienten mit HFmrEF lag prozentual in den meisten Kategorien zwischen den beiden anderen Herzinsuffizienz-Klassen. Patienten mit HFpEF waren überThe ewiegend weiblich, wiesen vergleichsweise am häufigsten eine komorbide Hypertonie oder ein Vorhofflimmern auf, während eine KHK deutlich seltener vorlag, als es in den anderen Herzinsuffizienz-Klassen der Fall war.
Die Prüfung der leitliniengerechten Pharmakotherapie bei HFrEF-Patienten ergab eine insgesamt gleichwertige Verschreibungshäufigkeit im geschlechtsspezifischen Vergleich der nach NYHA-Stadium indizierten Medikamentenklassen und Kombinationstherapien. Lediglich im NYHA-Stadium III konnte gezeigt werden, dass Männer signifikant häufiger mit einem Betablocker therapiert wurden. Weiterhin zeigte sich, bis auf wenige Ausnahmen, eine auch im nationalen und internationalen Vergleich hohe prozentuale Einnahme der stadienabhängig indizierten Medikamente. Die Einnahmerate von MRAs war vergleichsweise noch geringer als zu erwarten wäre, jedoch konnte das begleitende Vorliegen relevanter Kontraindikationen nicht zuverlässig genug erfasst werden, um die tatsächliche Versorgungslücke zu quantifizieren.
Die Analyse der Pharmakotherapie von HFmrEF- und HFpEF-Patienten zeigte, trotz bisher fehlender wissenschaftlicher Erkenntnisse zur optimalen medikamentösen Therapie dieser Patientengruppen, sehr ähnliche Einnahmehäufigkeiten der verschiedenen Substanzklassen im Vergleich zu den HFrEF-Patienten.
Die Therapie mit Devices war im Patientenkollektiv der HF-Bavaria Studie vergleichsweise selten und dabei häufiger bei männlichen Patienten vorzufinden. Die Analyse der leitliniengetreuen Indikationen von ICDs, CRTs und CRT-ICDs zu den tatsächlich implantierten Devices ergab Hinweise auf eine Unterversorgung vermittels apparativer Therapiemöglichkeiten.
Die Auswertung der HF-Bavaria Studie bestätigte die von uns erwartete Heterogenität und Komplexität der herzinsuffizienten Patienten in der niedergelassenen kardiologischen Betreuung. In dieser Arbeit konnte gezeigt werden, dass bedeutsame Unterschiede im Hinblick auf das Profil, den Verlauf und die Therapie von männlichen und weiblichen herzinsuffizienten Patienten bestehen. Die Therapieempfehlungen der Leitlinien richten sich trotz dieser Unterschiede vorrangig nach der Herzinsuffizienz-Klasse der Patienten. Bisher existierten in den Leitlinien vorrangig Therapieempfehlungen für Patienten mit einer HFrEF (und LVEF <40 %). Im Patientenkollektiv fanden sich jedoch zu 71 % Patienten mit einer LVEF ≥40 %. Dies bedeutet, dass für den Großteil der Patienten in unserer Studie bisher keine evidenzbasierten Behandlungsalgorithmen existieren, insbesondere zur Pharmakotherapie. Künftig sollte die Forschung vermehrt auf diese Evidenzlücken eingehen und idealerweise eine personalisierte Therapie ermöglichen.
Abschließend lässt sich feststellen, dass die leitliniengerechte Therapie der herzinsuffizienten Patienten in der niedergelassenen kardiologischen Versorgung in Bayern eine im nationalen und internationalen Kontext fortgeschrittene Qualität besitzt. Dennoch wurden erwartungsgemäß Möglichkeiten zur Qualitätsverbesserung im vorliegenden Projekt identifiziert.
In Schönbach wurden im Rahmen der Erschließung eines Neubaugebiets Reste einer frühneolithischen Siedlung auf der „Aindlinger Terrassentreppe“ gefunden, die im Areal der Ausgrabung u. a. acht Hausgrundrisse, Öfen und zahlreiche Schlitzgruben umfassten. Sechs der Hausgrundrisse lassen durch Überlagerungen auf eine gewisse Siedlungskontinuität mit mindestens zwei Phasen schließen. Die Häuser entsprechen grundsätzlich in Ausrichtung und Aufbau dem Typus der Zeitstellung. Etwas aus dem Rahmen fallen jedoch die unterdurchschnittliche Größe der Häuser und der Siedlungsstandort mit einer Lage von etwa 460 m üNN und außerhalb der üblichen Lössböden.
Die aktuellen Veränderungen der Arbeitswelt erfordern eine stetige Anpassung an immer neue Herausforderungen seitens der Arbeitgeber und Arbeitnehmer. Lebenslanges Lernen und damit eine kontinuierliche Weiterbildung der Mitarbeiter ist essentiell für Unternehmen, um auf dem schnelllebigen Arbeitsmarkt wettbewerbsfähig zu sein. Die Bereitschaft und die Motivation von Menschen, dazuzulernen, ist jedoch sehr unterschiedlich. Eine mögliche Erklärung dafür liefert die dispositionelle Zielorientierung, welche der Achievement-Goal-Theorie entstammt. Das Konstrukt beschreibt, ob Menschen eine individuelle Präferenz für Lernziele (z.B. Kompetenzzuwachs) oder Leistungsziele (z.B. gute Beurteilungen bekommen oder schlechte Beurteilungen vermeiden) haben. Neben den Persönlichkeitsaspekten konzentriert sich die Forschung im Rahmen der Achievement-Goal-Theorie auch auf den Einfluss der Umgebung auf Lern- und Leistungsprozesse. Die sogenannte arbeitsplatzbezogene Zielorientierung beschreibt die wahrgenommene Zielstruktur der Arbeitsumgebung und stellt das situationsbedingte Gegenstück der Dispositionen dar. Zahlreiche Befunde aus dem Bereich der pädagogischen Psychologie zu Zielstrukturen der Umgebung zeigen einen Einfluss auf beispielsweise Lernerfolg, Motivation, Selbstregulationsprozesse oder Leistung. Zielstrukturen im Arbeitskontext stellen hingegen ein bisher wenig beachtetes Konstrukt dar. Ausgehend von den aktuellen Befunden zu Zielstrukturen, könnte die arbeitsplatzbezogene Zielorientierung jedoch einen wichtigen Beitrag leisten, wenn es um die Frage geht, wie Mitarbeiter unterstützt und zu Lernprozessen angeregt werden können. Die Identifizierung von lern- und leistungsförderlichen Zielstrukturen der Arbeitsumgebung würde wertvolle Ansatzpunkte für die Mitarbeiterentwicklung in der betrieblichen Praxis liefern.
Im Rahmen von drei empirischen Studien wird der Relevanz der Zielorientierung im Arbeits-kontext nachgegangen. Neben der Überprüfung eines ins Deutsche übertragenen und angepassten Mess-instruments zur Erhebung der arbeitsplatzbezogenen Zielorientierung (Studie 1) steht vor allem die Untersuchung möglicher Einflüsse des Konstrukts auf arbeitsrelevante Variablen im Vordergrund, um förderliche Zielstrukturen zu identifizieren (Studie 1 & 2). Darüber hinaus werden erstmalig mögliche Person-Situation-Interaktionen in diesem Zusammenhang untersucht (Studie 2). Abschließend erfolgt eine Untersuchung möglicher Antezedenten der arbeitsplatzbezogenen Zielorientierung, woraus sich erste wichtige Anhaltspunkte für Interventionsmaßnahmen am Arbeitsplatz ableiten lassen (Studie 3).
Die Ergebnisse der Studien zeigen, dass insbesondere ein lernzielorientierter Arbeitsplatz förderlich für die untersuchten Variablen wie Lernerfolg, Leistung oder auch berufliche Selbstwirksamkeit ist. In Bezug auf die Interaktion von Person und Situation ergaben sich gemischte Befunde, die kein eindeutiges Interaktionsmuster aufweisen. Bei der Frage, wie ein lernzielorientierter Arbeitsplatz gefördert werden kann, erwies sich vor allem die Art und Weise, wie Führungskräfte mit Fehlern umgehen, als relevant. Die Studien liefern demnach wichtige erste Ansätze für theoretische und praktische Implikationen, wie Mitarbeiter in Lern- und Leistungsprozessen unterstützt werden können.
Zelluläre Resorption 3D-gedruckter Knochenimplantate auf Basis von Calciummagnesiumphosphaten

Für die Behandlung von Knochendefekten kritischer Größe gibt es heute eine Reihe von Therapiemöglichkeiten. Neuartige Ansätze mit Magnesiumphosphat- (MPC) und Calciummagnesiumphosphatzementen (CMPC) haben sich als echte Alternativen zu den etablierten Calciumphosphaten erwiesen.
Ziel war es, die Osteoklastogenese in vitro auf 3D-pulvergedrucktem CMPC und MPC zu induzieren und die zelluläre Resorption (zR) zu analysieren. Polystyrol (PS), Glas, β-TCP und Brushit-bildender Zement dienten als Referenzen.
Als Proben wurden Zemente der allgemeinen stöchiometrischen Summenformel CaxMg(3–x)(PO4)2 (x = 0; 0,25; 0,75; 3) verwendet, die Struvit oder Newberyit enthielten. Für die Osteoklastogenese wurden monozytenangereicherte PBMCs aus Buffy-Coat mittels dreifacher Dichtegradientenzentrifugation isoliert, auf die Prüfoberflächen ausgesät und über einen Zeitraum von 22 Tagen mit Zytokinen (M-CSF und RANKL) stimuliert. Die Interaktion der Zellen mit den Zementen bzw. PS/Glas wurde mittels TRAP-Färbung und -Aktivität, DNA- und Ionenkonzentrationen (Ca2+, Mg2+, PO43–, pH-Wert), Rasterelektronen-, Durchlicht-, Auflicht- und Fluoreszenzmikroskopie analysiert.
Auf den Struvit- und Newberyit-bildenden Zementen konnten keine für Osteoklasten typischen Riesenzellen nachgewiesen werden. Auf den Struvit-bildenden Zementen wurde deutlich mehr mononukleäre Zellen nachgewiesen wurden als auf den Newberyit-bildenden Zementen. Während die Freisetzung von Mg2+ und PO43– ausschließlich durch die chemische Degradation erfolgte, wurde Ca2+ zunächst adsorbiert und anschließend durch zR freigesetzt. Die erhöhte Ca2+-Adsorption im Vergleich zur Ca2+-Resorption führte insgesamt zu einer Calcium-Präzipitation.
Da lediglich auf β-TCP Resorptionslakunen beobachtet wurden, wird angenommen, dass auf den CMPC, MPC und Brushite-bildenden Zementen die zellvermittelte Ca2+-Freisetzung von den Präzipitaten ausging, die von Makrophagen auf den Zementen und/oder Riesenzellen auf den Wellplatten resorbiert wurden.
Die Schistosomiasisprävalenz und Krankheitslast in der Region Mwanza ist trotz staatlicher Maßnahmen anhaltend hoch. Zur Vorbereitung einer integralen Eliminationsstudie auf der Insel Ijinga sollte erfasst werden, ob die Bevölkerung über relevante Kenntnisse und Motivation verfügt, um sich an nachhaltiger Schistosomiasiskontrolle zu beteiligen. Zudem sollten lokal priorisierte Maßnahmen, sowie hemmende und unterstützende Faktoren identifiziert werden, um geplante Maßnahmen bestmöglich an die lokalen Begebenheiten anzupassen.
Von Februar bis März 2016 wurde eine quantitative Befragung von 356 Schulkindern, sowie 393 Erwachsenen durchgeführt. Anhand logistischer Regression wurden Faktoren ermittelt die mit einem relevanten Kenntnisstand assoziiert sind. Unter Schulkindern wurden des Weiteren Faktoren bestimmt, die mit einer hohen Motivation an Therapiekampagnen teilzunehmen, einhergehen.
Nur ein geringer Teil der Bevölkerung (17% der Kinder und 48% der Erwachsenen) verfügte über ein Mindestmaß an relevantem Wissen, um sich an nachhaltiger Schistosomiasiskontrolle zu beteiligen. Assoziierte Faktoren waren bei den Kindern die Informationsquelle Schule, der Besuch der 6. und 7. Klassen und eine zurückliegende Diagnose. Bei Erwachsenen bestand ein Zusammenhang zu verschiedenen Informationsquellen (Schule, Gesundheitswesen, lokale Medien), zum Alter, Bildungsstand und zur Präsenz von Symptomen. Nur 40% der Kinder hatten eine hohe Motivation an Therapiekampagnen teilzunehmen. Assoziierte Faktoren waren die wahrgenommene eigene Vulnerabilität und Schwere der Erkrankung, eine zurückliegende Diagnose, sowie eine erwartete Heilung durch Praziquantel.
Die aktuelle Wissensvermittlung in der 6. und 7. Schulklasse erfolgt zu spät und zu punktuell, um über einen theoretischen Kenntnisstand hinaus zur Verhaltensänderung zu führen. Eine systematische Aufnahme von Schistosomiasis in verschiedene Sektoren könnte das Bewusstsein in der Bevölkerung stärken. Zur nachhaltigen Einbindung der Bevölkerung ist ein partizipativer Ansatz, priorisierte WASH Interventionen und eine Stärkung des dezentralen Gesundheitssystems zur frühen Fallfindung und Therapie sinnvoll.
Das Glioblastom (GBM) ist der häufigste maligne primäre Hirntumor im Erwachsenenalter und geht mit einer infausten Prognose einher. Die Standardtherapie bei Erstdiagnose besteht aus Tumorresektion gefolgt von kombinierter Radiochemotherapie mit Temozolomid nach Stupp-Schema. Eine neue Therapieoption stellen die Tumor Treating Fields (TTFields) in Form lokal applizierter elektrischer Wechselfelder dar. Mit dem Einsatz der TTFields kann durch Störung der mitotischen Abläufe die Zellproliferation von Tumorzellen gehemmt und dadurch das Gesamtüberleben im Vergleich zur alleinigen Radiochemotherapie nachweislich deutlich verlängert werden. Auch verschiedene Chemotherapeutika, die bereits klinisch eingesetzt werden, greifen in den Ablauf der Mitose ein. So auch die Zytostatika Vincristin (VIN) und Paclitaxel (PTX), die durch einen gegensätzlichen Mechanismus durch Destabilisierung bzw. Stabilisierung von Mikrotubulistrukturen ihre Wirkung entfalten. Die Frage, ob eine Verstärkung dieser Wirkung durch den kombinierten Einsatz mit TTFields erreicht werden kann, wurde in dieser Arbeit an den beiden GBM-Zelllinien U87 und GaMG untersucht.
Zunächst wurde mit dem xCELLigence-Systems über eine Real-Time-Impedanzmessung für diese beiden Chemotherapeutika jeweils die mittlere effektive Dosis (EC50-Wert), bei der ein halbmaximaler Effekt auftritt, spezifisch für jede Zelllinie bestimmt. Diese betrug bei VIN durchschnittlich 200nM für die Zelllinie U87 bzw. 20nM für die Zelllinie GaMG und lag für PTX bei 60nM für beide Zelllinien. Mit diesen Dosierungen wurden die beiden Zelllinien allein und in Kombination mit TTFields über 72h behandelt. Anschließend wurde die Zellproliferation analysiert und mit unbehandelten Tumorzellen verglichen.
Während jeder Behandlungsarm einzeln eine signifikante Wirkung gegenüber der unbehandelten Vergleichsgruppe zeigte, hatte weder die Kombination von TTFields mit VIN noch mit PTX in den untersuchten Dosierungen einen zusätzlichen signifikanten Nutzen.
Es besteht weiterer Forschungsbedarf zum kombinierten Einsatz von TTFields mit anderen Therapieformen.
