Pemphigus vulgaris (PV) ist eine blasenbildende Autoimmunerkrankung der Haut. Ein wesentliches Charakteristikum der Erkrankung sind Autoantikörper, welche gegen die humanen Zell-Adhäsionsmoleküle Desmoglein (Dsg) 3 und 1 gerichtet sind und zu zunehmender Zell-Dissoziation der Keratinozyten führen (Akantholyse). Neben der Dsg3-Reorganisation sind zytoskelettale Veränderungen in Form einer ZK-Retraktion und einer Reorganisation des Actin-Zytoskeletts als ein wichtiges Merkmal akantholytischer Zellen beschrieben worden. Dennoch ist der zeitliche Verlauf und die funktionelle Relevanz dieser zytoskelettalen Veränderungen im Vergleich zu anderen Prozessen, wie der Dsg3-Reorganisation oder der Zell-Dissoziation, unklar. In dieser Arbeit wurde daher die Rolle der ZK-Filamente und der Actinfilamente für die PV-Pathogenese untersucht. Inkubation von kultivierten Keratinozyten mit PV-IgG resultierte in einer ZK-Retraktion, welche eng mit dem Beginn der Dsg3-Reorganisation und der Zell-Dissoziation korrelierte. Weiterhin fand sich eine Abhängigkeit der PV-IgG-induzierten ZK-Retraktion und der Zell-Dissoziation von der p38MAPK-Signalkaskade, während die Beteiligung der p38MAPK an der Dsg3-Reorganisation von untergeordneter Rolle zu sein scheint. Übereinstimmend dazu führte eine Überexpression von E-Cadherin zu einer Hemmung der p38MAPK-Aktivierung, der ZK-Retraktion und der Zell-Dissoziation, so dass den Cadherinen eine übergeordnete Rolle in der Vermittlung der PV-Pathogenese zuzukommen scheint. Neben einer ZK-Retraktion zeigten die Zellen als Reaktion auf eine Inkubation mit PV-IgG auch wesentliche Reorganisationen der Actinfilamente, welche ebenfalls eng mit der Dsg3-Reorganisation und der Zell-Dissoziation korrelierten. Darüber hinaus interferierte die pharmakologische Modulation des Actin-Zytoskeletts mit den PV-IgG-Effekten. So führte eine Stabilisierung der Actinfilamente zu einer Reduktion sowohl der Dsg3-Reorganisation als auch der Zell-Dissoziation, während eine Zerstörung der Filamente die Effekte verstärkte. Zur Unterstützung dieser Ergebnisse wurde die Rolle des Actins für die durch Rho-GTPasen vermittelte Hemmung von PV-IgG-Effekten untersucht. Eine Aktivierung der Rho-GTPasen führte neben einer Hemmung PV-IgG-vermittelter Effekte auch zu einer Verstärkung des kortikalen Actin-Rings, während eine Hemmung der Actin-Polymerisation die protektiven Effekte der Rho-GTPasen-Aktivierung aufheben konnte. Zusammenfassend lässt sich sagen, dass die Ergebnisse dieser Arbeit eine übergeordnete Rolle sowohl der desmosomalen als auch der klassischen Cadherine für die PV-Pathogenese zeigen. Daneben scheint auch der Actin-Reorganisation eine wesentliche Position zuzukommen. Die ZK-Retraktion hingegen scheint, zumindest im Bezug auf die Dsg3-Reorganisation, sekundär zu sein, trägt aber möglicherweise im Anschluss an eine p38MAPK-Aktivierung wesentlich zum Verlust der Zell-Zell-Adhäsion bei.
On the Fragility Index

The Fragility Index captures the amount of risk in a stochastic system of arbitrary dimension. Its main mathematical tool is the asymptotic distribution of exceedance counts within the system which can be derived by use of multivariate extreme value theory. Thereby the basic assumption is that data comes from a distribution which lies in the domain of attraction of a multivariate extreme value distribution. The Fragility Index itself and its extension can serve as a quantitative measure for tail dependence in arbitrary dimensions. It is linked to the well known extremal index for stochastic processes as well the extremal coefficient of an extreme value distribution.
Das Multiple Myelom ist eine Neoplasie die (fast) ausschließlich im Knochenmark lokalisiert ist. Verschiedene lösliche Faktoren und Zellinteraktionen sind für das Wachstum der Myelomzellen notwendig. Gute Untersuchungen zum Support der Myelomzellen gibt es für die Knochenmarkstromazellen. In dieser Arbeit wird gezeigt, dass auch Osteoklasten zum Zellwachstum der Myelomzellen beitragen. Es gibt Hinweise darauf dass dies z.T. durch Zell-zell-Interaktionen vermittelt wird. In der Analyse der Signalwege (MAPK-ERK-, STAT-3-, NF-Kappa-B- und AKT-Signalweg)zeigt sich ein unterschiedliches Aktivierungsmuster bei Support durch Osteoklasten oder Knochenmarkstromazellen. Weitere Signalwege sind wahrscheinlich an der Unterstützung des Wachstums der Myelomzellen beteiligt. Diese bedürfen einer weiteren Analyse.
Untersuchung der diagnostischen Wertigkeit eines standardisierten Algorithmus in der Differentialdiagnostik der Hyponatriämie, der diagnostischen Wertigkeit von Copeptin (Spaltprodukt von ADH) als ergänzender Diagnosemarker der Hyponatriämie und Vergleich mit der diagnostischen Wertigkeit bekannter Volumenparameter.
Die Arbeitsgruppe Zimmer am Institut für Klinische Biochemie und Pathobiochemie der Universität Würzburg detektierte im Gen der Dihydrolipoamid-Dehydrogenase (DLD) eine bisher nicht bekannte Mutation, die für die Entwicklung einer familiären Form der dilatativen Kardiomyopathie (DCM) verantwortlich ist. Die DLD spielt als Teil von mitochondrialen Enzymkomplexen eine wichtige Rolle im Energie- und Aminosäurestoffwechsel der Zelle. Mutationen im DLD-Gen führen dabei meist zu neurologischen Syndromen mit Erscheinungsbildern wie mentaler Entwicklungsverzögerung, Krampfanfällen und spastischen Bewegungsstörungen. Fälle von Herzinsuffizienz und frühkindlicher Hypertropher Kardiomyopathie wurden ebenfalls beschrieben. Von den zahlreichen Gendefekten, die als Auslöser für die dilatative Kardiomyopathie bekannt sind, ist bisher noch keiner im Gen der DLD beschrieben worden. Vielmehr sind DCM-Mutationen in Genen zu finden, die für muskelspezifische Proteine kodieren. Dadurch führen sie oft zu einer Beeinflussung der Kraftübertragung im Sarkomer. Die durch die Arbeitsgruppe Zimmer beschriebene DLD-Mutation wurde in einer portugiesischen Großfamilie entdeckt, in welcher das Auftreten der dilatativen Kardiomyopathie über mehrere Generationen hinweg zu verfolgen ist. Ähnliche Fälle außerhalb dieser Familie sind nicht bekannt. Demnach gibt es keine Daten, die die Häufigkeit DCM-assoziierter Mutationen im Gen der Dihydrolipoamid-Dehydrogenase beschreiben. Die vorliegende Arbeit beschäftigte sich damit weitere Zusammenhänge zwischen genetischen Alterationen im DLD-Gen und dem Auftreten einer DCM aufzudecken. In diesem Rahmen wurden DCM-Patienten, die erwiesenermaßen an einer familiären Form dieser Erkrankung leiden, gezielt auf Veränderungen in den Exons des DLD-Gens untersucht. Insgesamt wurden die Exons von 88 Patienten auf das Vorhandensein heterozygoter Mutationen überprüft. Hierfür wurden PCR-Produkte, die die jeweiligen Exons enthielten, mit Hilfe der Denaturing High-Performance Liquid Chromatography (DHPLC) untersucht. Diese Methode ermöglicht eine hochsensitive und gleichermaßen äußerst spezifische Detektion heterozygoter Mutationen. Auffällige Ergebnisse wurden anschließend mittels Sequenzierung verifiziert. Insgesamt wurden bei elf Patienten fünf unterschiedliche Mutationen nachgewiesen. Es handelte sich um vier bereits bekannte Einzelnukleotid-Polymorphismen und eine bisher nicht beschriebene Mutation. Dabei lagen vier Mutationen in nicht näher bezeichneten Intron-Bereichen, eine Mutation in einer 3‘ Spleißstelle und eine weitere Mutation in der 3‘ UTR (untranslated region) der mRNA. Somit befanden sich also einige Mutationen an für die Regulation der Genfunktion strategisch wichtigen Positionen. Da es sich dabei um bekannte Polymorphismen handelte, wurde mit Hilfe der Daten des HapMap Projekts überprüft, ob es bereits Hinweise für eine klinische Assoziation gab. Die Daten zeigten, dass bisher keiner der hier detektierten SNPs mit klinischen Erscheinungsbildern in Verbindung gebracht werden konnte. Es gab auch keine Hinweise dafür, dass die erfassten SNPs im Patientenkollektiv häufiger vorkamen als in der Normalbevölkerung. Einer der hier beschriebenen Mutationen war nicht in den verwendeten Datenbanken aufgeführt. Daher kann ein pathologischer Einfluss dieser Mutation zwar nicht ausgeschlossen werden, erscheint aber aufgrund ihrer Lage in einem weit vom Exon entfernten Intron-Bereich nicht offensichtlich. Es ließen sich also für keine der detektierten Mutationen pathogene Eigenschaften nachweisen. In Protein-kodierenden Sequenzbereichen konnten keine Mutationen nachgewiesen werden. Abschließend lässt sich also sagen, dass eine Assoziation zwischen Mutationen im DLD-Gen und dem Auftreten einer familiären DCM im Rahmen dieser Arbeit nicht bestätigt werden konnte. Es ist jedoch möglich, dass DCM-assoziierte Mutationen im DLD-Gen nur äußerst selten auftreten oder aber die durch die Arbeitsgruppe Zimmer detektierte Mutation im DLD-Gen die bisher einzige ist, die mit DCM in Verbindung gebracht werden kann. Um diese Frage zu klären müssen Untersuchungen mit größeren Patientenkollektiven angeschlossen werden.
Konzepte zur skalierbaren Realisierung von effizienten, halbleiterbasierten Einzelphotonenquellen

Dem Einsatz niederdimensionaler Nanostrukturen als optisch aktives Medium wird enormes Potential vorausgesagt sowohl in den klassischen optoelektronischen Bauteilen (wie z.B. Halbleiterlasern) als auch in optischen Bauteilen der näachsten Generation (z.B. Einzelphotonenquellen oder Quellen verschränkter Photonenpaare). Dennoch konnten sich quantenpunktbasierte Halbleiterlaser, abgesehen von einigen wenigen Ausnahmen (QDLaser inc.), im industriellen Maßstab bisher nicht gegen Bauelemente mit höherdimensionalen Quantenfilmen als optisch aktivem Element durchsetzen. Deshalb scheint der Einsatz von Quantenpunkten (QPen) in nichtklassischen Lichtquellen gegenwärtig vielversprechender. Um jedoch solche Bauteile bis zur letztendlichen Marktreife zu bringen, müssen neben der starken Unterdrückung von Multiphotonenemission noch wesentliche Grundvoraussetzungen erfüllt werden: In dieser Arbeit wurden grundlegende Studien durchgeführt, welche insbesondere dem Fortschritt und den Problemen der Effizienz, des elektrischen Betriebs und der Skalierbaren Herstellung der Photonenqullen dienen sollte. Zum Einen wurden hierfür elektrisch betriebene Einzelphotonenquellen basierend auf gekoppelten QP-Mikroresonatoren realisiert und de ren Bauteileffizienz gezielt optimiert, wobei konventionelle selbstorganisierte InAs-QPe als aktives Medium eingesetzt wurden. Für die skalierbare Integration einzelner QPe in Mikroresonatoren wurde des Weiteren das gesteuerte QP-Wachstum auf vorstrukturierten Substraten optimiert und auf diese Art ortskontrollierte QPe in Bauteile integriert. Für die Realisierung hocheffizienter, elektrisch gepumpter inzelphotonenquellen wurde zunächst das Wachstum von binären InAs-QPen im Stranski-Krastanov-Modus optimiert und deren optische Eigenschaften im Detail untersucht. Durch das Einbringen einer Schicht von Siliziumatomen nahe der QP-Schicht konnten die Emitter negativ geladen werden und der helle Trionenzustand der QPe als energetischer Eigenzustand des Systems zur effizienten Extraktion einzelner Photonen ausgenutzt werden. Durch die Integration dieser geladenen QPe in elektrisch kontaktierte, auf Braggspiegel basierte Mikrotürmchen konnten Einzelphotonenquellen realisiert werden, in denen gezielt Licht-Materie- Wechselwirkungseffekte zur Steigerung der Bauteileffizienz ausgenutzt wurden. Basierend auf theoretischen Überlegungen wurde die Schichtstruktur soweit optimiert, dass letztendlich experimentell eine elektrisch gepumpte Einzelphotonenquelle mit einer Photonenemissionsrate von 47 MHz sowie einer zuvor unerreichten Bauteileffizienz von 34 % im Regime der schwachen Licht-Materie-Kopplung demonstriert werden konnte. Da Effekte der Licht-Materie-Wechselwirkung zwischen QP und Resonator neben der spektralen Resonanz ebenfalls von der relativen Position von Resonator und QP zueinander abhängen, ist eine Kombination von positionierten QPen und Bauteilausrichtung nahezu unumg¨anglich für die skalierbare, deterministische Herstellung von Systemen aus perfekt angeordnetem Emitter und Resonator. Deshalb wurden bestehende Konzepte zum geordneten Wachstum von QPen weiterentwickelt: Hierbei wurde geordnetes InAs-QP-Wachstum mit Perioden realisiert, die vergleichbare Abmessungen wie optische Resonatoren aufweisen, also Nukleationsperioden zwischen 500 nm und 4 μm. Durch ein genaues Anpassen der Wachstums- und Prozessbedingungen konnte des Weiteren die Bildung von QP-Molekülen auf den Nukleationsplätzen nahezu unterdrückt beziehungsweise gesteuert werden. Durch eine systematische Optimierung der optischen Eigenschaften der QPe konnten Emitter mit Einzelquantenpunktlinienbreiten um 100 μeV realisiert werden, was eine Grundvoraussetzung zur Studie ausgeprägter Licht-Materie-Wechselwirkungseffekte in Mikroresonatoren darstellt. Letztendlich konnten durch die Integration derartiger QPe in optisch sowie elektrisch betriebene Mikroresonatoren erstmals Bauteile realisiert werden, welche einige der prinzipiellen, an eine Einzelphotonenquelle gestellten Anforderungen erfüllen. Insbesondere konnten deutliche Signaturen der schwachen Licht-Materie-Kopplung einzelner positionierter QPe in photonische Kristallresonatoren, Mikroscheibenresonatoren sowie Mikrotürmchenresonatoren festgestellt werden. Darüberhinaus konnte an einem spektral resonanten System aus einem positionierten QP und der Grundmode eines Mikrotürmchenresonators eindeutig Einzelphotonenemission unter optischer Anregung demonstriert werden. Ebenfalls konnten Mikrotürmchenresonatoren mit integrierten positionierten QPen erstmals elektrisch betrieben werden und somit die Grundvoraussetzung für eine der skalierbaren Herstellung effizienter Einzelphotonenquellen geschaffen werden.
Understanding the mechanisms of fragmentation within silicate melts is of great interest not only for material science, but also for volcanology, particularly regarding molten fuel coolant-interactions (MFCIs). Therefore edge-on hammer impact experiments (HIEs) have been carried out in order to analyze the fracture dynamics in well defined targets by applying a Cranz-Schardin highspeed camera technique. This thesis presents the corresponding results and provides a thorough insight into the dynamics of fragmentation, particularly focussing on the processes of energy dissipation. In HIEs two main classes of cracks can be identified, characterized by completely different fracture mechanisms: Shock wave induced “damage cracks” and “normal cracks”, which are exclusively caused by shear-stresses. This dual fracture situation is taken into account by introducing a new concept, according to which the crack class-specific fracture energies are linearly correlated with the corresponding fracture areas. The respective proportionality constants - denoted “fracture surface energy densities” (FSEDs) - have been quantified for all studied targets under various constraints. By analyzing the corresponding high speed image sequences and introducing useful dynamic parameters it has been possible to specify and describe in detail the evolution of fractures and, moreover, to quantify the energy dissipation rates during the fragmentation. Additionally, comprehensive multivariate statistical analyses have been carried out which have revealed general dependencies of all relevant fracture parameters as well as characteristics of the resulting particles. As a result, an important principle of fracture dynamics has been found, referred to as the “local anisotropy effect”: According to this principle, the fracture dynamics in a material is significantly affected by the location of directed stresses. High local stress gradients cause a more stable crack propagation and consequently a reduction of the energy dissipation rates. As a final step, this thesis focusses on the volcanological conclusions which can be drawn on the basis of the presented HIE results. Therefore fragments stemming from HIEs have been compared with natural and experimental volcanic ash particles of basaltic Grimsvötn and rhyolitic Tepexitl melts. The results of these comparative particle analyses substantiate HIEs to be a very suitable method for reproducing the MFCI loading conditions in silicate melts and prove the FSED concept to be a model which is well transferable to volcanic fragmentation processes.
The material system of interest in this thesis are II-VI-semiconductors. The first part of this thesis focuses on the formation of self-assembled CdSe-based quantum dots (QD) on ZnSe. The lattice constants of ZnSe and CdSe differ as much as about 7\% and therefore a CdSe layer grown on top of ZnSe experiences a huge strain. The aspired strain relief constitutes in the self-assembly of QDs (i.e. a roughened layer structure). Additionally, this QD layer is intermixed with Zn as this is also a possibility to decrease the strain in the layer. For CdSe on ZnSe, in Molecular Beam Epitaxy (MBE), various QD growth procedures were analysed with respect to the resulting Cd-content of the non-stoichiometric ternary (Zn,Cd)Se. The evaluation was performed by Raman Spectroscopy as the phonon frequency depends on the Cd-content. The second part of the thesis emphasis on the interface properties of n-ZnSe on n-GaAs. Different growth start procedures of the ZnSe epilayer may lead to different interface configurations with characteristic band-offsets and carrier depletion layer widths. The analysis is mainly focused on the individual depletion layer widths in the GaAs and ZnSe. This non-destructive analysis is performed by evaluating the Raman signal which comprises of phonon scattering from the depleted regions and coupled plasmon-phonon scattering from regions with free carriers.
This thesis concerned the quantification of cell adhesion molecules (CAM) in and on thin hydrogel films as surface modification of biomaterials. The established and well characterized, per se inert NCO-sP(EO-stat-PO) hydrogel system which allows the easy and reproducible bioactivation with peptides was used as basis for this thesis. Two methods can be used to functionalize the coatings. Ligands can either be mixed into the prepolymer solution in prior to layer formation (mix-in method), or freshly prepared coatings can be incubated with ligand solution (incubation method). Divided into three major parts, the first part of the thesis dealt with the concentration of ligands in the bulk hydrogel, whereas the second part of the thesis focused on the surface sensitive quantification of CAMs at the biointerface. The results were correlated with cell adhesion kinetics. The third part of this thesis investigated the biochemical and the structural mimicry of the extracellular matrix (ECM). ECM proteins were presented via sugar-lectin mediated binding and cell behavior on these surfaces was analyzed. Cell behavior on three-dimensional fibers with identical surface chemistry as the coatings in the previous sections of the thesis was analyzed and correlated with the amount of peptide used for bioactivation. Overall, the main question of this work was ‘How much?’ regarding maximal as well as optimal ligand concentrations for controlled cell-hydrogel interactions. The focus in the first practical part of this thesis was to analyze the amount of ligands in NCO-sP(EO-stat-PO) hydrogels using classical quantification methods. Coatings in 96-well plates as well as on glass were functionalized with GRGDS and 125I-YRGDS for radioisotopic detection (Chapter 3). Using the incubation method for functionalization, a maximal ligand binding using peptide concentrations of 600 µg/mL could be determined. When functionalization was introduced via the mix-in method, a clear tendency for higher ligand concentrations with increasing ligand to prepolymer ratio was observed, but no maximal ligand binding could be detected with a ligand to prepolymer ratio of 2/1 being the highest ratio investigated. This ratio of 2/1 was not exceeded to ensure that complete crosslinking of the hydrogel was not affected. In Chapter 4, a fluorinated amino acid and an iodinated peptide were immobilized to the hydrogels using the mix-in method and were detected by X-ray photoelectron spectroscopy (XPS) and time-of-flight secondary ion mass spectrometry (TOF-SIMS). In these measurements, maximal ligand binding was detected for a ligand to prepolymer ratio of 1/1. Higher ligand to prepolymer ratios did not result in any significant increase in ligand concentrations in the surface near regions of the crosslinked hydrogels. To address the question of how many ligands were actually accessible for cell interaction at the interface, surface sensitive quantification methods were applied in the second part of this thesis. For the quantification with surface plasmon resonance (SPR) and surface acoustic wave technology (SAW) (Chapter 5), the hydrogel coating procedure needed to be transferred onto cystamine functionalized gold surfaces. Characterization with ellipsometry and atomic force microscopy (AFM) revealed inhomogeneous cystamine binding to the activated surfaces, which resulted in inhomogeneous coatings. Nevertheless, it could be shown that SPR as well as SAW were suitable methods for the surface sensitive quantification of the ligand concentration on NCO-sP(EO-stat-PO) hydrogels. Non-functionalized coatings resisted non-specific serum as well as streptavidin (SA) adsorption. Coatings functionalized with biocytin and GRGDSK-biotin introduced specific SA binding that was dependent on the biotin concentration at the surface. Additionally, enzyme linked immunosorbent assay (ELISA) and enzyme linked lectin assay (ELLA) (Chapter 6) were applied to coatings in 96-well plates and on glass. Coatings were functionalized with the model molecule biocytin, the biotinylated peptide GRGDSK-biotin, the ECM protein fibronectin (FN), as well as the carbohydrates N-acetylglucosamine (GlcNAc) and N-acetyllactosamine (LacNAc). All ligands could be successfully detected with antibodies or SA via ELISA or ELLA. Maximal GRGDSK-biotin binding to the hydrogel coatings on glass was achieved at a peptide to prepolymer ratio of 1/5, which was used as reference value in Chapter 8. Last but not least, cell adhesion (Chapter 7) was quantified depending on the GRGDS concentration on hydrogel coatings on glass. Maximal adhesion of primary human dermal fibroblast (HDF) was observed at GRGDS to prepolymer ratios of 1/5, when adherent cells were counted on life cell images. Quantification of adherent cells using the CASY® cell counter revealed maximal HDF adhesion at molar ligand to prepolymer ratios of 1/2. However, cell vitality detected by intracellular enzyme activities was not dependent on the GRGDS concentration. Cells which managed to adhere were vital regardless of the amount of ligands present. Additionally, adhesion of fibroblasts from the murine cell line NIH L929 was analyzed by counting on life cell images. These cells, being much smaller than the HDF cells, needed higher GRGDS to prepolymer ratios (2/1) for proper cell adhesion. All quantification methods applied to analyze hydrogels which were functionalized by the mix-in method in Chapter 3, 4, 6 and 7, were compared in Chapter 8. Radiodetection gave information about the ligand concentrations throughout the whole hydrogel and no maximal amount of ligands could be detected when increasing the peptide to prepolymer ratio. In contrast, XPS and TOF-SIMS which only penetrated the surface near regions of the coating, a maximal ligand binding to the hydrogel was detected for 1/1 ratios. SPR and SAW were not included in this comparison, as the coatings on gold need to be optimized first. The two surface sensitive quantification methods (ELISA and HDF adhesion) could give information about the quantity of peptide which was sterically available for SA or cell binding. With these methods, maximal SA and cell binding was detected at ratios of 1/5. These results underline the importance of carefully compare the different methods. Beside ligand quantification on hydrogels, the third part of this thesis was concerned with the biochemical and structural mimicry of the ECM by advanced ECM engineering to design biomimetic biomaterials that are better accepted by cells and tissue. The subject of Chapter 9 was the biomimetic and flexible presentation of the ECM protein FN. FN was attached via sugar-lectin mediated binding to NCO-sP(EO-stat-PO) hydrogels. The build-up of the covalently immobilized sugar poly-N-acetyllactosamine (polyLacNAc), the subsequent non-covalent binding of the fungal galectin His6CGL2, and FN could be elegantly proven by fluorescent staining on coatings which were functionalized with the sugar by micro contact printing (MCP). Further experiments were carried out on build-ups, where polyLacNAc was immobilized on the hydrogel by incubation. Optimal parameters for the layer build-up were determined by ELLA/ELISA. Only the complete build-up induced proper adhesion of HDFs. Compared to tissue culture polystyrene (TCPS), cells adhered and spread faster on the biomimetic surfaces. The flexible presentation of FN allowed HDFs to rearrange homogenously immobilized FN into fibrillar structures, which seemed not to be possible when FN was adsorbed on glass or covalently bound directly to the hydrogel coatings. This new approach of a flexible and biomimetic presentation of an ECM protein allows new ways to design biomaterials with best possible cell-material interactions. The work described in Chapter 10 focused on the structural mimicry of the fibrous ECM structures by electrospinning of synthetic, bioactive, and degradable fibers. Poly(D,L-lactide-co-glycolide) (PLGA) and NCO-sP(EO-stat-PO) were electrospun out of one solution in an easy one-step preparation resulting in fibers with an ultrathin inert hydrogel layer at the surface. By adding GRGDS to the solution prior to electrospinning, specifically interacting fibers could be obtained. In comparison to PLGA, the adsorption of bovine serum albumin (BSA) could be reduced by 99.2%. As a control, the non-active peptide GRGES was immobilized to the fiber. These fibers did not allow cell adhesion, showing that the integrity of the hydrogel coated fibers was not affected by the immobilization of peptides. HDF adhesion was obtained by functionalization with GRGDS, leading to the adhesion, spreading, and proliferation of HDFs. Also mesenchymal stem cells (MSC) could adhere to GRGDS functionalized fibers. Additionally, for ligand quantification, the ELISA technique was successfully transferred to fiber substrates. To highlight the potential of the approaches for the biochemical and structural mimicry of the ECM, the sugar polyLacNAc was immobilized on the PLGA/sP(EO-stat-PO) fibers followed by the subsequent layer build-up with His6CGL2 and FN. These fibers triggered HDF adhesion.
Die arrhythmogene rechtsventrikuläre Kardiomyopathie ARVC ist eine seltene Erkrankung des Herzmuskels. Die ARVC tritt oft familiär gehäuft auf und geht meist mit einer autosomal dominanten Vererbung einher. Durch molekulargenetische Untersuchungen konnten bisher neun Genmutationen identifiziert werden, davon ein Großteil in Bestandteilen der kardialen Desmosomen, am häufigsten im Plakophilin-2-Gen. Das Protein Plakophilin 2 ist Bestandteil kardialer Desmosomen und gehört zur Familie der Armadillo-Proteine. Es wird vermutet, dass Veränderungen im Protein Plakophilin 2 zu Schädigungen der Zell-Zell-Verbindungen führen. Der Verlust der Myocytenadhäsion führt zum Zelltod und regionalen Fibrosierung. Da Mutationen im PKP-2-Gen am häufigsten in den westlichen Ländern für die familiär auftretende ARVC verantwortlich sind, entschieden wir uns für das PKP-2-Gen als Kandidatengen für Familie A, die für ein signifikantes Ergebnis einer Kopplungsanalyse zu klein war. Bei der direkten Sequenzierung der 14 Exons des PKP2-Gens, konnte auf Exon 11 von Patient II-2 ein Einzelnucleotidpolymorphismus SNP identifiziert werden, der sich allerdings auch in gesunder Kontroll-DNA bestätigte und somit als nicht krankheitsrelevant gewertet werden konnte. Die restriktive Kardiomyopathie RCM ist ebenfalls eine sehr seltene Herzmuskelerkrankung. Sie ist durch eine Verminderung der diastolischen Dehnbarkeit der Ventrikel charakterisiert. Die RCM kann im Rahmen systemischer Erkrankungen auftreten oder genetisch bedingt sein. Bisher wurden 6 Mutationen im kardialen Troponin-I-Gen als Ursache identifiziert. Troponin I gehört zusammen mit Troponin C und T zum kardialen Troponinkomplex und ist für die Regulation der Ca2+-abhängigen Kontraktion der kardialen Myocyten verantwortlich. Kommt es zu Veränderungen im Troponin I, wird die physiologische Relaxation des myokardialen Gewebes gestört. Da Mutationen im TNNI3-Gen häufig an der Pathogenese der familiären RCM beteiligt sind, untersuchten wir bei Familie B das TNNI3-Gen durch Direktsequenzierung. Dabei wurden bei der Sequenzierung des Exon 2 des erkrankten Kindes III-1 vier zusätzliche, intronisch gelegene Basen auf einem Strang entdeckt, die zu einer Verschiebung des Leserasters führten. Die Kontrolle der Auffälligkeit durch die Sequenzierung des DNA-Abschnitts bei der ebenfalls erkrankten Mutter II-1, der Tante II-3 und des gesunden Vaters II-2 zeigten, dass die vier zusätzlichen Basen vom gesunden Vater II-2 auf das Kind III-1 vererbt wurden und somit nicht mit der monogen vererbten Krankheit korrelieren. Die Charakterisierung der genetischen Ursachen von familiär bedingten Kardiomyopathien bringt weitere Erkenntnisse der pathophysiologischen Mechanismen der Erkrankungen. Dies ist die Voraussetzung zur Entwicklung und Verbesserung von präventiven, diagnostischen und therapeutischen Maßnahmen. Sollte sich bei Patienten, die an einer idiopathischen Kardiomyopathie erkrankt sind, eine genetische Ursache finden, so ist es für Verwandte empfehlenswert sich ebenfalls einer klinischen und genetischen Untersuchung zu unterziehen, um im Falle eines positiven Ergebnisses rechtzeitig Komplikationen der Erkrankung vermeiden und eine präventive Therapie einleiten zu können.
Type 1 diabetes is an autoimmune disease that leads to the destruction of insulin-producing pancreatic beta cells and consequently to hyperglycemia. In the last 60 years, the prevalence of type 1 diabetes has been increasing constantly and is predicted to continue rising. About 80% of the disease risk is attributable to the genetic variation. Thanks to genome wide association studies the number of known disease-associated polymorphisms climbed from five to 53 in the last 10 years. As these studies reveal possible candidate genes but not underlying mechanisms we strove to take the next step and explore the association of two genes suggested by these studies with type 1 diabetes. As a method of choice we decided to use lentiviral RNAi in non obese diabetic (NOD) mice, a widely-used model for type 1 diabetes, introducing a shRNA directed against the target message into the genome of this mouse strain via a lentivirus. This allowed us to study the partial loss-of-function of the target gene within the context of diabetes, directly seeing its effect on autoimmune mechanisms. In this thesis we examined two different genes in this manner, Ctla4 and Clec16a. A type 1 diabetes associated polymorphism in the CTLA4 gene had been found to alter the splicing ratio of its variants soluble CTLA-4 (sCTLA-4) and full length CTLA-4, the associated allele producing less sCTLA-4 than the protective allele. We mimicked this effect by specifically targeting the sCtla4 mRNA via lentiviral RNAi in the NOD model. As a result we could confirm the reduction of sCTLA-4 to accelerate type 1 diabetes development. Furthermore we could show a function of sCTLA-4 in regulatory T cells, more specifically at least partly in their ability to modulate costimulation by antigen presenting cells. The second candidate gene, Clec16a was targeted with the shRNA in a way that was designed to knock down most splice variants. As the gene function and the effect of the associated SUMMARY 10 polymorphism was unknown, we reasoned this method to be feasible to investigate its role in type 1 diabetes. The knockdown of Clec16a in NOD mice resulted in an almost complete protection from diabetes development that could be attributed to T cells dysfunction. However, as expression patterns and a study of the Drospophila orthologue suggested a possible role of CLEC16A in antigen presentation we also examined antigen presenting cells in the thymus and periphery. Although we did not detect any effect of the knockdown on peripheral antigen presenting cells, thymic epithelial cells were clearly affected by the loss of CLEC16A, rendering them more activated and shifting the ratio of cortical to medullary epithelial cells in favor of cortical cells. We therefore suggest a role of CLEC16A in the selection of T cells, that needs, however, to be further investigated. In this thesis we provided a feasible and fast method to study function of genes and even of single splice variants within the NOD mouse model. We demonstrate its usefulness on two candidate genes associated with type 1 diabetes by confirming and unraveling the cause of their connection to the disease.
It has been proposed that different features of a face provide a source of information for separate perceptual and cognitive processes. Properties of a face that remain rather stable over time, so called invariant facial features, yield information about a face’s identity, and changeable aspects of faces transmit information underlying social communication such as emotional expressions and speech movements. While processing of these different face properties was initially claimed to be independent, a growing body of evidence suggests that these sources of information can interact when people recognize faces with whom they are familiar. This is the case because the way a face moves can contain patterns that are characteristic for that specific person, so called idiosyncratic movements. As a face becomes familiar these idiosyncratic movements are learned and hence also provide information serving face identification. While an abundance of experiments has addressed the independence of invariant and variable facial features in face recognition, little is known about the exact nature of the impact idiosyncratic facial movements have on face recognition. Gaining knowledge about the way facial motion contributes to face recognition is, however, important for a deeper understanding of the way the brain processes and recognizes faces. In the following dissertation three experiments are reported that investigate the impact familiarity of changeable facial features has on processes of face recognition. Temporal aspects of the processing of familiar idiosyncratic facial motion were addressed in the first experiment via EEG by investigating the influence familiar facial movement exerts on event-related potentials associated to face processing and face recognition. After being familiarized with a face and its idiosyncratic movement, participants viewed familiar or unfamiliar faces with familiar or unfamiliar facial movement while their brain potentials were recorded. Results showed that familiarity of facial motion influenced later event-related potentials linked to memory processes involved in face recognition. The second experiment used fMRI to investigate the brain areas involved in processing familiar facial movement. Participants’ BOLD-signal was registered while they viewed familiar and unfamiliar faces with familiar or unfamiliar idiosyncratic movement. It was found that activity of brain regions, such as the fusiform gyrus, that underlie the processing of face identity, was modulated by familiar facial movement. Together these two experiments provide valuable information about the nature of the involvement of idiosyncratic facial movement in face recognition and have important implications for cognitive and neural models of face perception and recognition. The third experiment addressed the question whether idiosyncratic facial movement could increase individuation in perceiving faces from a different ethnic group and hence reduce impaired recognition of these other-race faces compared to own-race faces, a phenomenon named the own-race bias. European participants viewed European and African faces that were each animated with an idiosyncratic smile while their attention was either directed to the form or the motion of the face. Subsequently recognition memory for these faces was tested. Results showed that the own-race bias was equally present in both attention conditions indicating that idiosyncratic facial movement was not able to reduce or diminish the own-race bias. In combination the here presented experiments provide further insight into the involvement of idiosyncratic facial motion in face recognition. It is necessary to consider the dynamic component of faces when investigating face recognition because static facial images are not able to provide the full range of information that leads to recognition of a face. In order to reflect the full process of face recognition, cognitive and neural models of face perception and recognition need to integrate dynamic facial features as a source of information which contributes to the recognition of a face.
Ziel dieser Arbeit war es, die telepathologische Zusammenarbeit mit einem Krankenhaus eines medizinisch unterentwickelten Landes zu überprüfen und zu bewerten. Für die Untersuchungen wurden die Daten von 545 Patienten aus Peramiho/Tansania übermittelten Fälle analysiert. Dabei wurden die telepathologisch erstellten Diagnosen anhand der nachträglich nach Würzburg übersandten Paraffinblöckchen der Gewebeproben und dort erfolgten Nachbegutachtung nach deutschen Standards überprüft. Die Ergebnisse zeigten, dass der Einsatz telepathologischer Techniken für die Unterstützung unterentwickelter Länder geeignet ist und die Qualität der medizinischen Versorgung langfristig verbessert.
The Nucleotide Excision Repair (NER) pathway is able to remove a vast diversity of structurally unrelated DNA lesions and is the only repair mechanism in humans responsible for the excision of UV induced DNA damages. The NER mechanism raises two fundamental questions: 1) How is DNA damage recognition achieved discriminating damaged from non damaged DNA? 2) How is DNA incision regulated preventing endonucleases to cleave DNA non specifically but induce and ensure dual incision of damaged DNA? Thus, the aim of this work was to investigate the mechanisms leading from recognition to incision of damaged DNA. To decipher the underlying process of damage recognition in a prokaryotic model system, the intention of the first part of this work was to co crystallize the helicase UvrB form Bacillus caldotenax together with a DNA substrate comprising a fluorescein adducted thymine as an NER substrate. Incision assays were performed to address the question whether UvrB in complex with the endonuclease UvrC is able to specifically incise damaged DNA employing DNA substrates with unpaired regions at different positions with respect to the DNA lesion. The results presented here indicate that the formation of a specific pre incision complex is independent of the damage sensor UvrA. The preference for 5’ bubble substrate suggests that UvrB is able to slide along the DNA favorably in a 5’ → 3’ direction until it directly encounters a DNA damage on the translocating strand to then recruit the endonuclease UvrC. In the second part of this work, the novel endonuclease Bax1 from Thermoplasma acidophilum was characterized. Due to its close association to archaeal XPB, a potential involvement of Bax1 in archaeal NER has been postulated. Bax1 was shown to be a Mg2+ dependent, structure specific endonuclease incising 3’ overhang substrates in the single stranded region close to the ssDNA/dsDNA junction. Site directed mutagenesis of conserved amino acids was employed to identify putative active site residues of Bax1. In complex with the helicase XPB, however, incision activity of Bax1 is altered regarding substrate specificity. The presence of two distinct XPB/Bax1 complexes with different endonuclease activities indicates that XPB regulates Bax1 incision activity providing insights into the physical and functional interactions of XPB and Bax1.
Die Magnet-Resonanz (MR)-Bildgebung ist mit vielfältigen Anwendungen ein nicht mehr wegzudenkendes Instrument der klinischen Diagnostik geworden. Dennoch führt die stark limitierte Messzeit häufig zu einer Einschränkung der erzielbaren räumlichen Auflösung und Abdeckung, einer Beschränkung des Signal-zu-Rauschverhältnis (Signal-to-Noise Ratio) (SNR) sowie einer Signalkontamination durch benachbartes Gewebe. Bereits bestehende Methoden zur Reduktion der Akquisitionszeit sind die partielle Fourier (PF)-Bildgebung und die parallele Bildgebung (PPA). Diese unterscheiden sich zum einen im Schema zur Unterabtastung des k-Raums und zum anderen in der verwendeten Information zur Rekonstruktion der fehlenden k-Raum-Daten aufgrund der beschleunigten Akquisition. Während in der PPA die unterschiedlichen Sensitivitäten einer Mehrkanal-Empfangsspule zur Bildrekonstruktion verwendet werden, basiert die PF-Bildgebung auf der Annahme einer langsamen Variation der Bildphase. Im ersten Abschnitt dieser Arbeit wurde das Konzept der Virtuellen Spulendekonvolutions (Virtual Coil Deconvolution) (VIDE)-Technik vorgestellt, das das gleiche Schema der Unterabtastung des k-Raums wie die konventionelle PPA verwendet, aber anstelle der Spulensensitivität die Bildphase als zusätzliche Information zur Herstellung der fehlenden Daten der beschleunigten Bildgebung verwendet. Zur Minimierung der Rekonstruktionsfehler und der Rauschverstärkung in der VIDE-Technik wurde ein optimiertes Akquisitionsschema entwickelt. Die Kombination der PPA und PF-Bildgebung zur Beschleunigung der MR-Bildgebung wird durch das unterschiedliche Unterabtastschema erschwert. Wie Blaimer et al. in ihrer Arbeit gezeigt haben, kann das Prinzip der VIDE-Technik auf Mehrkanal-Spulen übertragen werden, sodass mit dieser Methode die PPA und die PF-Bildgebung optimal vereint werden können. Dadurch kann die Rauschverstärkung aufgrund der Spulengeometrie ohne zusätzliche Messungen deutlich reduziert werden. Obwohl die Abtastung des k-Raums in der MR-Bildgebung sehr variabel gestaltet werden kann, wird bis heute nahezu ausschließlich die regelmäßige k-Raum-Abtastung in der klinischen Bildgebung verwendet. Der Grund hierfür liegt, neben der schnellen Rekonstruktion und der einfachen Gestaltung der Variation des Bild-Kontrasts, in der Robustheit gegen Artefakte. Allerdings führt die regelmäßige k-Raum-Abtastung zu einer hohen Signalkontamination. Die Optimierung der SRF durch nachträgliches Filtern führt jedoch zu einem SNR-Verlust. Die dichtegewichtete (DW-) Bildgebung ermöglicht die Reduktion der Signal-Kontamination bei optimalem SNR, führt aber zur einer Reduktion des effektiven Gesichtsfelds (FOV) oder einer Erhöhung der Messzeit. Letzteres kann durch eine Kombination der PPA und DW-Bildgebung umgangen werden. Der zweite Teil dieser Arbeit befasste sich mit neuen Aufnahme- und Rekonstruktionsstrategien für die DW-Bildgebung, die eine Erhöhung des FOVs auch ohne Einsatz der PPA erlauben. Durch eine Limitierung der minimalen k-Raum-Abtastdichte konnte durch eine geringfügige Reduktion des SNR-Vorteils der DW-Bildgebung gegenüber der kartesischen, gefilterten Bildgebung eine deutliche Verringerung der Artefakte aufgrund der Unterabtastung in der DW-Bildgebung erreicht werden. Eine asymmetrische Abtastung kann unter der Voraussetzung einer homogenen Bildphase das Aliasing zusätzlich reduzieren. Durch die Rekonstruktion der DW-Daten mit der Virtuelle Spulendekonvolution für die effektive DW-Bildgebung (VIDED)-Bildgebung konnten die Artefakte aufgrund der Unterabtastung eliminiert werden. In der 3d-Bildgebung konnte durch Anwendung der modifizierten DW-Bildgebung eine Steigerung des FOVs in Schichtrichtung ohne Messzeitverlängerung erreicht werden. Die nicht-kartesische k-Raum-Abtastung führt im Fall einer Unterabtastung zu deutlich geringeren, inkohärenten Aliasingartefakten im Vergleich zur kartesischen Abtastung. Durch ein alternierendes DW-Abtastschema wurde eine an die in der MR-Mammografie verwendete Spulengeometrie angepasste k-Raum-Abtastung entwickelt, das bei gleicher Messzeit die räumliche Auflösung, das SNR und das FOV erhöht. Im dritten Teil dieser Arbeit wurde die Verallgemeinerung der DW-Bildgebung auf signalgewichtete Sequenzen, d.h. Sequenzen mit Magnetisierungspräparation (Inversion Recovery (IR), Saturation Recovery (SR)) sowie Sequenzen mit einer Relaxation während der Datenaufnahme (Multi-Gradienten-Echo, Multi-Spin-Echo) vorgestellt, was eine Steigerung der Bildqualität bei optimalem SNR erlaubt. Die Methode wurde auf die SR-Sequenz angewendet und deren praktischer Nutzen wurde in der Herz-Perfusions-Bildgebung gezeigt. Durch die Verwendung der in dieser Arbeit vorgestellten Technik konnte eine Reduktion der Kontamination bei einem SNR-Gewinn von 16% im Vergleich zur konventionellen, kartesischen Abtastung bei gleicher Messzeit erreicht werden.
Der Hefepilz Candida albicans gehört zu den fakultativ pathogenen Infektionserregern und ist Teil der natürlichen Mikroflora der Schleimhäute des Verdauungs- und Urogenitaltraktes der meisten gesunden Menschen. Ist das Gleichgewicht der Flora gestört, kann es zu oberflächlichen Mykosen kommen, wie z.B. der oropharyngealen Candidiasis (Mundsoor), die in der Regel durch die Gabe eines Antimykotikums in wenigen Tagen zu behandeln sind. In seltenen Fällen kann es auch zu schwerwiegenden Infektionsverläufen bis hin zu lebensbedrohlichen systemischen Mykosen kommen. Hauptsächlich immunsupprimierte Patienten, wie z.B. AIDS-Patienten oder Personen, die kürzlich einer Organ- oder Knochenmarkstransplantation unterzogen wurden, leiden häufig an oberflächlichen C. albicans-Infektionen. Insbesondere bei wiederkehrenden Infektionen ist der Pilz in der Lage, gegen das häufig verabreichte Medikament Fluconazol eine Resistenz zu entwickeln. Ein wichtiger Mechanismus dieser Resistenzentwicklung ist die Überexpression von Effluxpumpen, die das Medikament aus der Zelle heraustransportieren. Zwei Arten von Effluxpumpen, die eine Rolle in der Resistenzentwicklung in C. albicans spielen, konnten bisher identifiziert werden, die ABC (ATP binding cassette)-Transporter Cdr1 und Cdr2 sowie der MFS (major facilitator superfamily)-Transporter Mdr1. Der Zinc-Cluster Transkriptionsfaktor Mrr1 spielt eine wichtige Rolle in der Regulation der MDR1-E¬ffluxpumpe. Er kontrolliert die MDR1-Expression in Anwesenheit induzierender Substanzen und sogenannte "gain-of-function" Mutationen in MRR1 konnten als die Ursache der konstitutiven MDR1-Hochregulierung und der "Multidrug-Resistance" in C. albicans identifiziert werden. In dieser Arbeit konnte ein Ortholog zu MRR1 aus C. albicans in Candida dubliniensis, einer zu C. albicans nahe verwandten Hefe, identifiziert werden. Es wurde gezeigt, dass in den untersuchten klinischen und in vitro generierten Fluconazol-resistenten C. dubliniensis-Stämmen ebenfalls gain-of-funcion Mutationen in MRR1 die MDR1-Überexpression und eine Resistenz bewirken. Die Ergebnisse demonstrieren, dass der Transkriptionsfaktor Mrr1 eine wichtige Rolle in der Entwicklung der Resistenz in diesen humanpathogenen Pilzen spielt. Bisher ist nicht bekannt, wie der Zinc-Cluster Transkriptionsfaktor MRR1 durch induzierende Substanzen oder gain-of-function Mutationen aktiviert wird. Um zu verstehen, wie die Mrr1- Aktivität reguliert wird, wurden in dieser Arbeit durch Deletionsstudien funktionelle Domänen des Transkriptionsfaktors identifiziert. Um einen besseren Einblick in die Regulation der MDR1-vermittelten Resistenz in C. albicans zu bekommen, wurde in dieser Arbeit die gegenseitige Abhängigkeit von Mrr1 und Cap1 bzw. Upc2 in Bezug auf die MDR1-Expression untersucht. Es wurden ChIP-on-chip Analysen und Transkriptionsprofile mit aktiviertem Mrr1 durchgeführt, um direkte Targets von Mrr1 zu identifizieren. Mit der vorliegenden Arbeit wurde ein wichtiger Beitrag zum Verständnis der Entwicklung der Multidrug-Resistenz in C. albicans geleistet. E¬ffluxpumpen und deren Regulatoren stellen in der Bekämpfung von C. albicans-Infektionen ein interessantes Angriffsziel für die Entwicklung neuer Medikamente und die Weiterentwicklung bereits vorhandender Antimykotika dar.
This study was conducted to determine the influence of different stress factors on the honeybee Apis mellifera. The investigation was motivated by previous experiments that suggested the existence of an unspecific defense mechanism causing a generalized change of flight behavior after the onset of different diseases. This mechanism is thought to impede the ability of flight bees to return to their respective colonies thereby removing the disease from the colony over time. During the last years, the existence of such a “suicidal behavior” was supported by further studies. Thus, an unnoticed, potentially highly effective defense mechanism of social insects was revealed whose spectrum of activity and physiological basics require further investigation. Suggesting that the reaction by the bees is unspecific to different diseases as well as to other potential stress factors, this study was designed to investigate the influence of pathogens, insecticides, and different brood rearing temperatures on different parameters like lifespan, foraging activity, and foraging trip duration of worker bees.
Das Ziel dieser Arbeit war die Evaluierung von morphologischen und funktionellen Techniken zur Untersuchung der Lunge am Niederfeld MRT bei Patienten mit Mukoviszidose. Patienten mit Mukoviszidose und lungengesunde Probanden wurden an einem Niederfeld-MRT (0,2 Tesla) mittels coronaren TrueFISP, FLASH 2D und FLASH 3D Sequenzen untersucht. T1 und T2*-Messungen wurden während Atmung von Raumluft und Atmung von 100 % Sauerstoff durchgeführt und die Parameterkarten pixelweise berechnet. Die für die Lungenbildgebung am Niederfeld-MRT optimierten 2D und 3D FLASH Sequenzen zeigten ein signifikant besseres Signalverhalten als die Standardsequenz TrueFISP. Zur Beurteilung der Parenchymveränderungen wurde ein MR-Score in Anlehnung an den Chrispin-Norman-Score angewandt. Es zeigte sich eine gute Korrelation zwischen dem MR-Score der FLASH-Sequenzen und dem etablierten CN-Score der konventionellen Bildgebung mit einer geringen Interobservariabiliät für die 2D und 3D FLASH Sequenzen. Schließlich konnte eine O2-gestütze funktionelle Bildgebung der Lunge bei Patienten mit Mukoviszidose am offenen Niederfeld-MRT etabliert werden. Es zeigten sich gute Korrelationen zwischen der relativen Änderung der T1 Relaxationszeit und der spirometrisch bestimmten Lungenfunktion. Ein solcher Zusammenhang konnte für die T2*-Messungen nicht hergestellt werden. Aufgrund der Patientenfreundlichkeit ist diese Technik insbesondere für die Untersuchung von Kindern geeignet.
Hintergrund: Ein bidirektionaler Zusammenhang zwischen Diabetes mellitus und Parodontitis wird durch zahlreiche wissenschaftliche Arbeiten bestätigt. Bislang weitgehend unerforscht bleibt jedoch die Frage, ob die beiden pathophysiologisch verschiedenen Krankheitsbilder des Typ-1- und des Typ-2-Diabetes bezüglich Häufigkeit und Ausmaß parodontaler Erkrankungen Divergenzen aufzeigen. Zielstellung: Ziel der vorliegenden Untersuchung war es die parodontale Gesundheit eines Patientenkollektivs alterskorrelierter Diabetiker mit inadäquat eingestelltem Blutzucker in Abhängigkeit vom vorliegenden Diabetestyp (Typ-1 bzw. Typ-2) zu evaluieren. Material und Methoden: 101 insulinpflichtige Diabetiker (Typ-1: n=47, Typ-2: n=54.), welche in der Klinik Saale im Rahmen einer stationären Reha-Maßnahme behandelt wurden, nahmen an der Studie teil. Dabei mussten sie folgende Einschlusskriterien erfüllen: Alter 35-60J., HbA1c≥7%, Insulintherapie, Nichtraucher, ≥10 natürliche Zähne, keine parodontale Therapie oder syste¬mische Antibiose in den letzten 6 Monaten. Erfasst wurden die Zahl der natürlichen Zähne sowie an den Zähnen 16,21,24,36,41,44 (Ramfjord-Zähne) die Parameter Taschentiefe, Attachmentniveau, Gingiva-Index (GI) nach Löe&Silness und Plaque-Index (PI) nach Quigley&Hein. Basierend auf Attachmentniveau und Sondie-rungstiefen wurden die Patienten zudem gemäß den Kriterien der CDC/AAP-Arbeitsgruppe zur Klassifizierung parodontaler Erkrankungen einer von drei parodontalen Erkrankungskategorien (gesund-mild/moderat/schwer) zugeordnet. Des Weiteren wurden den ärztlichen Entlassungsbriefen der Klinik Saale zahlreiche charakteristische Daten entnommen, wie Patientenalter, Krankheitsdauer, HbA1c, Ausmaß von Folge- und Begleiterkrankungen sowie insbesondere auch BMI und CRP. Ergebnisse: Trotz erheblich kürzerer Krankheitsdauer (11,7 vs. 20,3 Jahre) und bei vergleichbarer Altersstruktur (51,3 vs. 48,3 Jahre) zeigten Typ-2-Diabetiker gegenüber Typ-1-Diabetikern eine signifikant geringere Zahnzahl (24,5 vs. 26,2 Zähne; p<0,05), einen signifikant erhöhten GI-Score (4,8 vs. 2,9; p<0,001), einen signifikant erhöhten PI-Score (8,8 vs. 6,4), einen signifikant höheren Anteil schwerer Parodontalerkankungen gemäß CDC/AAP-Kriterien (40,7% vs. 23,4%; p<0,05), signifikant höhere CRP-Werte (0,66 vs.0,31 mg/dl; p<0,001) und einen signifikant höheren BMI (37,08 vs. 27,05 kg/m²; p<0,001). Die HbA1c-Werte beider Gruppen waren nicht statistisch signifikant unterschiedlich (8,88 vs. 8,39%). Schlussfolgerung: Im Vergleich von Typ-1- und insulinpflichtigen Typ-2-Diabetikern mit annähernd vergleichbarer Altersstruktur und Diabeteseinstellung zeigen Typ-2-Diabetiker, trotz deutlich kürzerer Diabetesdauer, signifikant häufiger Symptome einer schweren Parodontitis. Dies deutet darauf hin, dass neben Hyperglykämien weitere für Typ-2-Diabetes typische ätiologische Faktoren, insbesondere subklinische Inflammationen im Rahmen des Metabolischen Syndroms, für die erhöhte Prävalenz parodontaler Erkrankungen unter Diabetikern von Bedeutung sind. Für detaillierte Aussagen sind weitere gezielte klinische Studien notwendig.
