Background
The anti-SLAMF7 monoclonal antibody, elotuzumab (elo), plus lenalidomide (len) and dexamethasone (dex) is approved for relapsed/refractory MM in the U.S. and Europe. Recently, a small phase 2 study demonstrated an advantage in progression-free survival (PFS) for elo plus pomalidomide (pom)/dex compared to pom/dex alone and resulted in licensing of this novel triplet combination, but clinical experience is still limited.
Purpose
To analyze the efficacy and safety of elo/pom/dex in a “real world” cohort of patients with advanced MM, we queried the databases of the university hospitals of Würzburg and Vienna.
Findings
We identified 22 patients with a median number of five prior lines of therapy who received elo/pom/dex prior to licensing within an early access program. Patients received a median number of 5 four-week treatment cycles. Median PFS was 6.4 months with 12-month and 18-month PFS rates of 35% and 28%, respectively. The overall response rate was 50% and 64% of responding patients who achieved a longer PFS with elo/pom/dex compared to their most recent line of therapy. Objective responses were also seen in five patients who had been pretreated with pomalidomide. Low tumor burden was associated with improved PFS (13.5 months for patients with ISS stage I/II at study entry v 6.4 months for ISS III), although this difference did not reach statistical significance. No infusion-related reactions were reported. The most frequent grade 3/4 adverse events were neutropenia and pneumonia.
Conclusion
Elo/pom/dex is an active and well-tolerated regimen in highly advanced MM even after pretreatment with pomalidomide.
Purpose
This study investigates the redislocation rate and functional outcome at a minimum follow-up of five years after medial patellofemoral ligament (MPFL) reconstruction with soft tissue patellar fixation for patella instability.
Methods
Patients were retrospectively identified and knees were evaluated for trochlea dysplasia according to Dejour, for presence of patella alta and for presence of cartilage lesion at surgery. At a minimum follow-up of five years, information about an incident of redislocation was obtained. Kujala, Lysholm, and Tegner questionnaires as well as range of motion were used to measure functional outcome.
Results
Eighty-nine knees were included. Follow-up rate for redislocation was 79.8% and for functional outcome 58.4%. After a mean follow-up of 5.8 years, the redislocation rate was 5.6%. There was significant improvement of the Kujala score (68.8 to 88.2, p = 0.000) and of the Lysholm score (71.3 to 88.4, p = 0.000). Range of motion at follow-up was 149.0° (115–165). 77.5% of the knees had patella alta and 52.9% trochlear dysplasia types B, C, or D. Patellar cartilage legions were present in 54.2%. Redislocations occurred in knees with trochlear dysplasia type C in combination with patella alta.
Conclusion
MPFL reconstruction with soft tissue patellar fixation leads to significant improvement of knee function and low midterm redislocation rate. Patients with high-grade trochlear dysplasia should be considered for additional osseous correction.
In an experiment with 114 children aged 9–12 years, we compared the ability to establish local and global coherence of narrative texts between auditory and audiovisual (auditory text and pictures) presentation. The participants listened to a series of short narrative texts, in each of which a protagonist pursued a goal. Following each text, we collected the response time to a query word that was either associated with a near or a distant causal antecedent of the final sentence. Analysis of these response times indicated that audiovisual presentation has advantages over auditory presentation for accessing information relevant for establishing both local and global coherence, but there are indications that this effect may be slightly more pronounced for global coherence.
Due to biased assumptions on the underlying ordinal rating scale in subjective Quality of Experience (QoE) studies, Mean Opinion Score (MOS)-based evaluations provide results, which are hard to interpret and can be misleading. This paper proposes to consider the full QoE distribution for evaluating, reporting, and modeling QoE results instead of relying on MOS-based metrics derived from results based on ordinal rating scales. The QoE distribution can be represented in a concise way by using the parameters of a multinomial distribution without losing any information about the underlying QoE ratings, and even keeps backward compatibility with previous, biased MOS-based results. Considering QoE results as a realization of a multinomial distribution allows to rely on a well-established theoretical background, which enables meaningful evaluations also for ordinal rating scales. Moreover, QoE models based on QoE distributions keep detailed information from the results of a QoE study of a technical system, and thus, give an unprecedented richness of insights into the end users’ experience with the technical system. In this work, existing and novel statistical methods for QoE distributions are summarized and exemplary evaluations are outlined. Furthermore, using the novel concept of quality steps, simulative and analytical QoE models based on QoE distributions are presented and showcased. The goal is to demonstrate the fundamental advantages of considering QoE distributions over MOS-based evaluations if the underlying rating data is ordinal in nature.
In the last few years, quantitative analysis of metabolites in body fluids using LC/MS has become an established method in laboratory medicine and toxicology. By preparing metabolite profiles in biological specimens, we are able to understand pathophysiological mechanisms at the biochemical and thus the functional level. An innovative investigative method, which has not yet been used widely in the forensic context, is to use the clinical application of metabolomics. In a metabolomic analysis of 41 samples of postmortem cerebrospinal fluid (CSF) samples divided into cohorts of four different causes of death, namely, cardiovascular fatalities, isoIated torso trauma, traumatic brain injury, and multi-organ failure, we were able to identify relevant differences in the metabolite profile between these individual groups. According to this preliminary assessment, we assume that information on biochemical processes is not gained by differences in the concentration of individual metabolites in CSF, but by a combination of differently distributed metabolites forming the perspective of a new generation of biomarkers for diagnosing (fatal) TBI and associated neuropathological changes in the CNS using CSF samples.
Purpose
Examine the effects of an 8-week yoga therapy on fatigue in patients with different types of cancer.
Methods
A total of 173 cancer patients suffering from mild to severe fatigue were randomly allocated to yoga intervention (n = 84) (IG) versus waitlist control group (CG) (n = 88). Yoga therapy consisted of eight weekly sessions with 60 min each. The primary outcome was self-reported fatigue symptoms. Secondary outcomes were symptoms of depression and quality of life (QoL). Data were assessed using questionnaires before (T0) and after yoga therapy for IG versus waiting period for CG (T1).
Results
A stronger reduction of general fatigue (P = .033), physical fatigue (P = .048), and depression (P < .001) as well as a stronger increase in QoL (P = .002) was found for patients who attended 7 or 8 sessions compared with controls. Within the yoga group, both higher attendance rate and lower T0-fatigue were significant predictors of lower T1-fatigue (P ≤ .001). Exploratory results revealed that women with breast cancer report a higher reduction of fatigue than women with other types of cancer (P = .016) after yoga therapy.
Conclusion
The findings support the assumption that yoga therapy is useful to reduce cancer-related fatigue, especially for the physical aspects of fatigue. Women with breast cancer seem to benefit most, and higher attendance rate results in greater reduction of fatigue.

Wissenschaftskompetenz ist eine Schlüsselqualifikation für jede ärztliche Tätigkeit und sollte ebenso wie die Auseinandersetzung mit Entscheidungsprozessen von Beginn an ins Medizinstudium integriert werden. Ziel der Studie war, die Themen der guten wissenschaftlichen Praxis und des wissenschaftlichen Fehlverhaltens zu vermitteln. Ferner wurde durch die methodische Intervention „Gruppendiskussion“ eine Reflexion im Kontext der wissenschaftlichen Angemessenheit herbeigeführt. Hierfür wurde der Situational Judgement Test (SJT) von den Studierenden (N = 743) (individuell und in der Gruppe) bearbeitet, und dessen Resultate wurden mit den Antworten von Expert/innen/en (N = 23) verglichen. Nach der Gruppendiskussion näherten sich die Ergebnisse in der Verteilung und Reihenfolge den Antwortmöglichkeiten der Expert/innen/en an. Jedoch tendierten die Studierenden signifikant häufiger zu jenen Antworten, die hilfesuchende, passive und verantwortungsübertragende Optionen bedeuteten. Insgesamt hat sich der SJT als didaktische Intervention bewährt. Die Studierenden setzten sich aktiv mit den Themen auseinander, eine Diskussion konnte angeregt und das eigene Verhalten kritisch reflektiert werden.
Die vorliegenden Arbeit behandelt VUV Valenz-Photoionisations-Experimente in der Gasphase. Zunächst wird die Photoionisation von stickstoffhaltigen Radikalen und deren Pyrolyseprodukten untersucht. Im Anschluss werden molekulare Biradikale betrachtet. Da in der Literatur bislang nur wenige solcher Biradikale als Intermediate experimentell zugänglich waren, war es das Ziel dieser Arbeit, neue reaktive Spezies dieser Substanzklassen in der Gasphase zu isolieren und deren Struktur, Eigenschaften und Reaktivität besser zu verstehen. Im Mittelpunkt stehen dabei Intermediate, die als echte Biradikale, Biradikaloide oder Triplett Carbene auftreten. Zu letzteren zählen das Methylbismut sowie die Pentadiinylidene. Biradikale bilden in Verbrennungsprozessen sehr effizient Ruß(vorläufer), was anhand des ortho-Benz-ins dargelegt wurde, indem dessen Pyrolyseprodukte charakterisiert und mögliche PAH-Bildungswege aufgezeigt wurden. Vakuum Flash Pyrolyse wurde verwendet, um in situ aus den geeigneten Vorläufermolekülen die radikalischen und biradikalischen Intermediate zu erzeugen. Während für biradikalische Zwischenstufen meist spezielle Verbindungen als Vorläufer synthetisiert werden müssen, waren die verwendeten Vorläufer für die stickstoffhaltigen Radikale kommerziell erhältlich. Die reaktiven Spezies wurden alle mittels monochromatischer VUV Synchrotronstrahlung an der Swiss Light Source in Villigen/ Schweiz ionisiert. Die Ionisationsereignisse wurden mit der Schwellenphotoelektronen-Photoionen-Koinzidenz (TPEPICO) Technik detektiert und ausgewertet. Anhand der resultierenden massenselektiven Schwellenphotoelektronenspektren wurden die Ionisierungsenergien der (Bi)radikale bestimmt und die Schwingungsstruktur der jeweiligen Kationen analysiert. Die erhaltenen Spektren und Daten wurden in Zusammenarbeit mit der theoretischen Chemie interpretiert.
Wichtige Erkenntnisse
• Es wurde die Ionisierungsenergie der 2-, 3- und 4-Picolylradikale auf 7.70\pm
0.02 eV, 7.59\pm
0.01 eV und 8.01\pm
0.01 eV bestimmt. Diese wurden in der Pyrolyse selektiv aus ihren zugehörigen Picolylaminen erzeugt. Zudem wurde analog zum Benzyl-Radikal für alle drei Radikale eine ausgeprägte Schwingungsprogression ermittelt, die der totalsymmetrischen Deformationsmode des aromatischen Rings entspricht.
• Die Picolyl-Radikale dissoziieren in der Pyrolyse thermisch zu weiteren Produkten. Die Fragmentierung verläuft dabei isomerenunabhängig über ein stickstoffhaltiges Siebenringintermediat, dem Azepinyl-Radikal. Der Fragmentierungsmechanismus wurde mit dem von Benzyl verglichen. Die gewonnenen Erkenntnisse haben Relevanz für Verbrennungsprozesse, beispielsweise von Biokraftstoffen.Im ersten Schritt entstehen vier Isomere, das Cyclopenta-1,4-dien-1-carbonitril, das Cyclopenta-1,3-dien-1-carbonitril, das 2-Ethynyl-1H-pyrrol und das3-Ethynyl-1H-pyrrol mit den zugehörigen Ionisierungsenergien von 9.25\pm
0.02 eV, 9.14\pm
0.02 eV, 7.99\pm
0.02 eV und 8.12\pm
0.02 eV. Durch einen zweiten H-Verlust konnte das Cyanocyclopentadienyl-Radikal mit einer Ionisierungsenergie für die zwei niedrigsten Zustände im Kation mit 9.07\pm
0.02 eV (T0) und 9.21\pm
0.02 eV (S1) untersucht werden. Weitere Pyrolyseprodukte, deren Ionisierungsenergien bereits literaturbekannt sind und die bestätigt wurden, sind das Cyclopentadienyl-Radikal, das Cyclopenta-1,3-dien, das Propargyl-Radikal, das Penta-1,3-diin und das Cyanopropenyl.
• Das ortho-Benz-in wurde pyrolytisch aus dem selbst synthetisierten Benzocyclobutendion erzeugt und ein Schwellenphotoelektronenspektrum frei von Störsignalen konnte aufgenommen werden. Mit Hilfe von Rechnungen aufCASPT2(11,14) Niveau, die neben dem elektronischen Übergang in den kationischen Grundzustand noch die Übergänge in zwei weitere angeregte kationische Zustände beinhalten, wurde die Ionisierungsenergie im Vergleich zu früheren Experimenten auf 9.51 eV revidiert. Eine verdrillte Geometrie für den kationischen Grundzustand konnte erstmals nachgewiesen werden. Zusätzlich wurden die offenkettigen Isomere cis- und trans-Hexa-1,5-diin-3-en im Spektrum detektiert und zugeordnet.
• Die Auftrittsenergien aus der DPI des Vorläufermoleküls Benzocyclobutendion betragen für den ersten CO-Verlust 9.62\pm
0.05 eV und für den zweiten CO-Verlust 12.14\pm
0.10 eV. Damit konnte über einen thermochemischen Kreisprozess eine Bindungsdissoziationsenergie für die Ph-CO Bindung im Benzoylkation von 2.52 eV berechnet werden.
• Verschiedenen Pyrolyseprodukte des ortho-Benz-ins, wie Ethin, Buta-1,3-diin, Benzol, Biphenylen und 2-Ethinylnaphthalin, werden entweder in bimolekularen Reaktionen gebildet oder ortho-Benz-in fragmentiert unimolekular zu diesen. Die beiden kompetitiven Reaktionspfade tragen zur PAH-Bildung des ortho-Benz-ins bei.
• Die Triplett-Carbene Pentadiinyliden, Methylpentadiinyliden und Dimethylpentadiinyliden wurden als Pyrolyseprodukt aus ihren zugehörigen Diazovorläufern identifiziert und die Ionisierungsenergien mit 8.36\pm
0.03 eV, 7.77\pm
0.04 eV und 7.27\pm
0.06 eV bestimmt. Jede Methylierung stabilisiert folglich das Carben. Zusätzlich konnte ein weiteres C5H2 Isomer, das 3-(Didehydrovinyliden)cyclopropen, mit einer Ionisierungsenergie von 8.60\pm
0.03 eV charakterisiert werden.
• Zwei bismuthaltige, reaktive Spezies, das Dimethylbismut-Radikal\cdot
BiMe2 (IE = 7.27\pm
0.04 eV) und das Methylbismut-Carben :BiMe(IE = 7.88\pm
0.02 eV) wurden als Pyrolyseprodukte aus dem BiMe3 identifiziert. Beide Verbindungen zeigen eine ausgeprägte Schwingungsstruktur, die der Bi-C Streckschwingung zugeordnet wurde. Weiterhin wurden elementares Bismut Bi und das Bismut-Dimer Bi2 nachgewiesen.
• Die homolytische Dissoziation der ersten Me2Bi-CH3 Bindung im BiMe3 wurde untersucht und eine BDE von 210\pm
7 kJ/ mol bestimmt. Sie liegt um +15 % bzw. +28 kJ/ mol über dem aus der Literatur abgeschätzten Wert.
Schicksal von Mikrokernen bzw. mikrokernhaltigen Zellen und Bedeutung von Mikrokernen als Biomarker
(2021)
Mikrokerne sind als wichtiger Biomarker in der Gentoxizitätsforschung seit langer Zeit etabliert und ihre Bildung ist mechanistisch gut verstanden, wohingegen das Mikrokernschicksal und die genaue Funktion von Mikrokernen in der Kanzerogenese unzureichend erforscht sind. Um das Schicksal von Mikrokernen und mikrokernhaltigen Zellen über einen längeren Zeitraum zu untersuchen, wurden HeLa-Zellen, die mit einem GFP-markierten Histon H2B transfiziert worden sind, mittels Lebendzellmikroskopie nach Behandlung mit verschiedenen gentoxischen Agenzien für 96 h untersucht. Parameter wie die Mitose- oder Zelltodrate wurden dabei ebenso wie das Schicksal der Mikrokerne dokumentiert. Während Persistenz und Reinkorporation von Mikrokernen häufig beobachtet wurden, waren Degradation und Auswurf von Mikrokernen selten bis gar nicht zu sehen. Auch konnte ein Teil der mikrokernhaltigen Zellen über mehrere Zellteilungen persistieren und proliferieren, wodurch die in Mikrokernen manifestierte chromosomale Instabilität unverändert bleiben kann. Ein eindeutiger Substanzeinfluss auf das Mikrokernschicksal konnte nicht ausgemacht werden. Extrusion sollte weiterhin durch Behandlung mit Hydroxyurea oder Cytochalasin B in Kombination mit gentoxischer Behandlung induziert werden, es wurde jedoch kein Effekt auf die Extrusionsrate beobachtet. Degradation wurde mittels γH2AX-Antikörperfärbung und transduziertem dsRed-markierten Autophagiemarker LC3B in HeLa-H2B-GFP-Zellen untersucht. Trotz erhöhter DNA-Degradation in Mikrokernen wurde nur selten eine Ko-Lokalisierung mit LC3B beobachtet. Dafür gab es in HeLa-H2B-GFP-Zellen, die zusätzlich mit dsRed markierten Kernmembranmarker Lamin B1 transduziert worden sind, Anzeichen für eine eingeschränkte Mikrokernmembranintegrität. Weiterhin wurden Zytokinese-Block Mikrokerntests nach Behandlung mit Thebain mit und ohne metabolische Aktivierung sowie Celecoxib und Celecoxibderivaten durchgeführt. Hierbei wurde nach Thebainbehandlung nur ohne metabolische Aktivierung und bei Anwesenheit von Zytotoxizität mehr Mikrokerne gefunden, während nach Behandlung mit Celecoxib und Celecoxibderivaten kein Anstieg beobachtet wurde. Zusätzlich wurde der Einfluss durch neurodegenerative Veränderungen auf Mundschleimhautzellen in zwei großen Kohorten untersucht, wobei keine Effekte auf die Häufigkeit von Mikrokernen oder mikrokernhaltigen Zellen zugeordnet werden konnten, während es teilweise bei Parametern, die auf Zytotoxizität hindeuten, zu Veränderungen kam. Es konnte insgesamt gezeigt werden, dass Mikrokerne und mikrokernhaltige Zellen zusätzlich zu ihrer Funktion als Biomarker über wenigstens mehrere Zellteilungen bestehen bleiben können. Auf diese Weise können sie z. B. über Chromothripsis zu einer beschleunigten Kanzerogenese führen, was zu einer schlechten Prognose für Krebspatienten führen kann.
Neisseria gonorrhoeae (GC) is a human specific pathogenic bacterium. Currently, N. gonorrhoeae developed resistance to virtually all the available antibiotics used for treatment. N. gonorrhoeae starts infection by colonizing the cell surface, followed by invasion of the host cell, intracellular persistence, transcytosis and exit into the subepithelial space. Subepithelial bacteria can reach the bloodstream and disseminate to other tissues causing systemic infections, which leads to serious conditions such as arthritis and pneumonia. A number of studies have well established the host-pathogen interactions during the initial adherence and invasion steps. However, the mechanism of intracellular survival and traversal is poorly understood so far. Hence, identification of novel bacterial virulence factors and host factors involved in the host-pathogen interaction is a crucial step in understanding disease development and uncovering novel therapeutic approaches. Besides, most of the previous studies about N. gonorrhoeae were performed in the conventional cell culture. Although they have provided insights into host-pathogen interactions, much information about the native infection microenvironment, such as cell polarization and barrier function, is still missing.
This work focused on determining the function of novel bacterial virulence factor NGFG_01605 and host factor (FLCN) in gonococcal infection. NGFG_01605 was identified by Tn5 transposon library screening. It is a putative U32 protease. Unlike other proteins in this family, it is not secreted and has no ex vivo protease activity. NGFG_01605 knockout decreases gonococcal survival in the epithelial cell. 3D models based on T84 cell was developed for the bacterial transmigration assay. NGFG_01605 knockout does not affect gonococcal transmigration.
The novel host factor FLCN was identified by shRNA library screening in search for factors that affected gonococcal adherence and/or internalization. We discovered that FLCN did not affect N. gonorrhoeae adherence and invasion but was essential for bacterial survival. Since programmed cell death is a host defence mechanism against intracellular pathogens, we further explored apoptosis and autophagy upon gonococcal infection and determined that FLCN did not affect apoptosis but inhibited autophagy. Moreover, we found that FLCN inhibited the expression of E-cadherin. Knockdown of E- cadherin decreased the autophagy flux and supported N. gonorrhoeae survival. Both non-polarized and polarized cells are present in the cervix, and additionally, E-cadherin represents different polarization properties on these different cells. Therefore, we established 3-D models to better understand the functions of FLCN. We discovered that FLCN was critical for N. gonorrhoeae survival in the 3-D environment as well, but not through inhibiting autophagy. Furthermore, FLCN inhibits the E-cadherin expression and disturbs its polarization in the 3-D models. Since N. gonorrhoeae can cross the epithelial cell barriers through both cell-cell junctions and transcellular migration, we further explored the roles FLCN and E-cadherin played in transmigration. FLCN delayed N. gonorrhoeae transmigration, whereas the knockdown of E-cadherin increased N. gonorrhoeae transmigration.
In summary, we revealed roles of the NGFG_01605 and FLCN-E-cadherin axis play in N. gonorrhoeae infection, particularly in relation to intracellular survival and transmigration. This is also the first study that connects FLCN and human-specific pathogen infection.
Virtual reality exposure therapy (VRET) is an effective cognitive-behavioral treatment for anxiety disorders that comprises systematic confrontations to virtual representations of feared stimuli and situations.
However, not all patients respond to VRET, and some patients relapse after successful treatment. One explanation for this limitation of VRET is that its underlying mechanisms are not yet fully understood, leaving room for further improvement.
On these grounds, the present thesis aimed to investigate two major research questions: first, it explored how virtual stimuli induce fear responses in height-fearful participants, and second, it tested if VRET outcome could be improved by incorporating techniques derived from two different theories of exposure therapy. To this end, five studies in virtual reality (VR) were conducted.
Study 1 (N = 99) established a virtual environment for height exposure using a Computer Automatic Virtual Environment (CAVE) and investigated the effects of tactile wind simulation in VR. Height-fearful and non-fearful participants climbed a virtual outlook, and half of the participants received wind simulation. Results revealed that height-fearful participants showed stronger fear responses, on both a subjective and behavioral level, and that wind simulation increased subjective fear. However, adding tactile wind simulation in VR did not affect presence, the user's sense of 'being there' in the virtual environment. Replicating previous studies, fear and presence in VR were correlated, and the correlation was higher in height-fearful compared to non-fearful participants.
Study 2 (N = 43) sought to corroborate the findings of the first study, using a different VR system for exposure (a head-mounted display) and measuring physiological fear responses. In addition, the effects of a visual cognitive distractor on fear in VR were investigated. Participants' fear responses were evident on both a subjective and physiological level---although much more pronounced on skin conductance than on heart rate---but the virtual distractor did not affect the strength of fear responses.
In Study 3 (N = 50), the effects of trait height-fearfulness and height level on fear responses were investigated in more detail. Self-rated level of acrophobia and five different height levels in VR (1 m--20 m) were used as linear predictors of subjective and physiological indices of fear. Results showed that subjective fear and skin conductance responses were a function of both trait height-fearfulness and height level, whereas no clear effects were visible for heart rate.
Study 4 (N = 64 + N = 49) aimed to advance the understanding of the relationship between presence and fear in VR. Previous research indicates a positive correlation between both measures, but possible causal mechanisms have not yet been identified. The study was the first to experimentally manipulate both presence (via the visual and auditive realism of the virtual environment) and fear (by presenting both height and control situations). Results indicated a causal effect of fear on presence, i.e., experiencing fear in a virtual environment led to a stronger sense of `being there' in the virtual environment. However, conversely, presence increased by higher scene realism did not affect fear responses. Nonetheless, presence seemed to have some effects on fear responding via another pathway, as participants whose presence levels were highest in the first safe context were also those who had the strongest fear responses in a later height situation. This finding indicated the importance of immersive user characteristics in the emergence of presence and fear in VR.
The findings of the first four studies were integrated into a model of fear in VR, extending previous models and highlighting factors that lead to the emergence of both fear and presence in VR. Results of the studies showed that fear responses towards virtual heights were affected by trait height-fearfulness, phobic elements in the virtual environment, and, at least to some degree, on presence. Presence, on the other hand, was affected by experiencing fear in VR, immersion---the characteristics of the VR system---and immersive user characteristics. Of note, the manipulations of immersion used in the present thesis, visual and auditory realism of the virtual environment and tactile wind simulation, were not particularly effective in manipulating presence.
Finally, Study 5 (N = 34) compared two different implementations of VRET for acrophobia to investigate mechanisms underlying its efficacy. The first implementation followed the Emotional Processing Theory, assuming that fear reduction during exposure is crucial for positive treatment outcome. In this condition, patients were asked to focus on their fear responses and on the decline of fear (habituation) during exposures. The second implementation was based on the inhibitory learning model, assuming that expectancy violation is the primary mechanism underlying exposure therapy efficacy. In this condition, patients were asked to focus on the non-occurrence of feared outcomes (e.g., 'I could fall off') during exposure. Based on predictions of the inhibitory learning model, the hypothesis for the study was that expectancy-violation-based exposure would outperform habituation-based exposure.
After two treatment sessions in VR, both treatment conditions effectively reduced the patients' fear of heights, but the two conditions did not differ in their efficacy. The study replicated previous studies by showing that VRET is an effective treatment for acrophobia; however, contrary to the assumption, explicitly targeting the violation of threat expectancies did not improve outcome. This finding adds to other studies failing to provide clear evidence for expectancy violation as the primary mechanism underlying exposure therapy. Possible explanations for this finding and clinical implications are discussed, along with suggestions for further research.
The Immune system exerts its response against invading pathogens via a cumulative, sequential cooperation of immune cells coordinated by their secreted products. Immune cells, such as macrophages and dendritic cells (DCs), express toll-like receptors (TLRs) to sense the presence of pathogens through pathogen-associated molecular patterns (PAMPs). The interaction of PAMPs with TLRs elicits a cytosolic signaling cascade that enhances the expression of genes to stimulate inflammation. Interleukin 1 receptor-associated kinase 2 (IRAK2) is a component of the TLR signaling pathway. IRAK2 transduces the TLR signal via a direct interaction with TNF receptor-associated factor 6 (TRAF6) and subsequent enhancement of its ubiquitination.
During my PhD thesis, I determined that a 55-amino acid long stretch at the C-terminal end of IRAK2 is important for TLR signaling. Overexpression of C-terminal truncated IRAK2 (IRAK2Δ55) in the murine macrophage cell line RAW 264.7 led to impaired CD40 expression after TLR4 stimulation by Lipopolysaccharide (LPS). I observed attenuated competency of IRAK2Δ55 in restoring a full TLR signaling response i.e. IL-6 secretion, NO production and CD40 expression in IRAK2-deficient RAW cells generated via CRISPR-Cas9 approach. Additionally, diminished TLR4 induced activation of nuclear factor κB (NF-κB) and extracellular signal related kinase (ERK) was observed with IRAK2Δ55 reconstituted RAW cells as compared to cell reconstituted with wildtype IRAK2.
IRAK2Δ55 reconstituted RAW cells also exhibited reduced TLR4-induced cell death and phosphorylation of receptor interacting protein kinase 3 (RIP3). Co-immunoprecipitation experiments in HEK 293T cells showed that IRAK2Δ55 was still able to bind to TRAF6 alike IRAK2 but failed to induce ubiquitination of TRAF6. In conclusion, the results suggest that the IRAK2 TRAF6 interaction is not sufficient to sustain full TLR signaling. An C-terminus-dependent unknown molecular mechanism is also involved.
Through my PhD work, I also analyzed a B cell lineage-specific HECTD1 knock-out mice. HECTD1 is an E3 ubiquitin ligase for various substrate proteins, such as heat shock protein 90 (HSP90), adenomatous polyposis coli and phosphatidylinositol phosphate kinase type 1 γ. Hsp90 regulates a variety of signaling molecules in NF-κB activation pathways which are essential for an optimal B cell response.
HECTD1-deficient pro-B cells developed normally into mature B cells. However, TLR4 stimulated HECTD1-deficient B cells displayed reduced immunoglobulin (Ig) production in in vitro cultures. In addition, mice with HECTD1-deficient B cells showed a diminished Ig response after nitrophenylacetyl-keyhole limpet hemocyanin immunization. Thus, HECTD1 is necessary for efficient Ig secretion.
The superfamily of G protein-coupled receptors (GPCRs) comprises more than 800 members, which are divided into five families based on phylogenetic analyses (GRAFS classification): Glutamate, Rhodopsin, Adhesion, Frizzled/Taste2 and Secretin. The adhesion G protein-coupled receptor (aGPCR) family forms with 33 homologs in Mammalia the second largest and least investigated family of GPCRs. The general architecture of an aGPCR comprises the GPCR characteristics of an extracellular region (ECR), a seven transmembrane (7TM) domain and an intracellular region (ICR). A special feature of aGPCRs is the extraordinary size of the ECR through which they interact with cellular and matricellular ligands via adhesion motif folds. In addition, the ECR contains a so-called GPCR autoproteolysis-inducing (GAIN) domain, which catalyzes autoproteolytic cleavage of the protein during maturation. This cleavage leads to the formation of an N-terminal (NTF) and a C-terminal fragment (CTF), which build a unit by means of hydrophobic interactions and therefore appear as a heterodimeric receptor at the cell surface. In the past, it has been shown that the first few amino acids of the CTF act as a tethered agonist (TA) that mediates the activation of the receptor through the interaction with the 7TM domain. However, the molecular mechanism promoting the TA-7TM domain interaction remains elusive. This work reveals a novel molecular mechanism that does not require the dissociation of the NTF-CTF complex to promote release of the TA and thus activation of the aGPCR. The introduction of bioorthogonal labels into receptorsignaling- relevant regions of the TA of various aGPCRs demonstrated that the TA is freely accessible within the intact GAIN domain. This suggests a structural flexibility of the GAIN domain, which allows a receptor activation independent of the NTF-CTF dissociation, as found in cleavage-deficient aGPCR variants. Furthermore, the present study shows that the cellular localization and the conformation of the 7TM domain depends on the activity state of the aGPCR, which in turn indicates that the TA mediates conformational changes through the interaction with the 7TM domain, which ultimately regulates the receptor activity. In addition, biochemical analyses showed that the GAIN domain-mediated autoproteolysis of the human aGPCR CD97 (ADGRE5/E5) promotes further cleavage events within the receptor. This suggests that aGPCRs undergo cleavage cascades, which are initialized by the autoproteolytic reaction of the GAIN domain. Thus, it can be assumed that aGPCRs are subject to additional proteolytic events. Finally, the constitutive internalization of the NTF and the CTF of E5 was demonstrated by various labeling methods. It was possible to label both fragments independently and to follow their subcellular location in vitro. In summary, these obtained results contribute to a better understanding about the molecular mechanisms of activity and signaling of aGPCRs.
Ziel der Untersuchung:
Verglichen wurden die räumlichen Abweichungen der Bohrpfade nach virtueller Planung von Schablonen geführten Trepanationen mit Hilfe der Softwaresysteme SicatEndo (SE) und coDiagnostiX (CDX) und der benötigte Arbeitsaufwand.
Material und Methode:
Basierend auf µCT-Datensätzen von humanen obliterierten Frontzähnen wurden identische Kunststoffzähne und acht Zahnmodelle (4 Ober-, 4 Unterkiefer) hergestellt. Es wurde jeweils ein DVT und ein Oberflächenscan angefertigt. Diese Datensätze (DICOM; STL) wurden in die Softwaresysteme importiert und fusioniert. Anschließend wurden die Bohrpfade für je 16 Probenzähne pro Software geplant. Mit Hilfe der erstellten Schablonen wurden alle Trepanationen an den im Phantomkopf fixierten Modellen von einem Behandler durchgeführt. Nach Erschließung des apikalen Wurzelkanalanteils wurde ein DVT angefertigt und mit dem präoperativen DVT überlagert. Die räumliche drei-dimensionale (3D) Abweichung zwischen virtuell geplantem und tatsächlichem Bohrpfad wurde über die Vektorlänge bestimmt und der Arbeitsaufwand anhand der Planungszeit und der Anzahl der Mausklicks pro Kiefer erfasst.
Ergebnisse:
Für die Trepanationen mit SE zeigten sich signifikant geringe Abweichungen an der Bohrerspitze vestibulär-oral [CDX 0,54mm ± 0,32mm; SE 0,12mm ± 0,11mm; p < 0.05], 3D [CDX 0,74mm ± 0,26 mm; SE 0,35mm ± 0,17mm; p < 0.05] und hinsichtlich des Winkels [CDX 1,57° ± 0,76°; SE 0,68° ± 0,41°; p < 0.05] als mit CDX. Für CDX war der Planungsaufwand signifikant geringer als für SE hinsichtlich Planungszeit [CDX Ø 10min 50sec; SE Ø 20min 28sec] und hinsichtlich der Anzahl der Klicks pro Kiefer [CDX Ø 107; SE Ø 341].
Zusammenfassung:
Beide Planungssysteme ermöglichen ausreichend präzise Schablonen geführte Bohrungen zur Erschließung apikaler Wurzelkanalanteile.
Stationäre Gasturbinen können von großer Bedeutung für die Verlangsamung des Klima-wandels und bei der Bewältigung der Energiewende sein. Für die Weiterentwicklung von Gasturbinen zu höheren Betriebstemperaturen und damit einhergehend zu höheren Wirkungs-graden werden berührungslose Messverfahren zur Ermittlung der Oberflächentemperatur von Turbinenschaufeln und der Gastemperatur der heißen Verbrennungsgase während des Be-triebs benötigt. Im Rahmen dieser Arbeit werden daher Methoden der berührungslosen Tem-peraturmessung unter Verwendung von Infrarotstrahlung untersucht.
Die berührungslose Messung der Oberflächentemperatur moderner Turbinenschaufeln muss aufgrund derer infrarot-optischer Oberflächeneigenschaften im Wellenlängenbereich des mitt-leren Infrarots durchgeführt werden, in welchem die Turbinenbrenngase starke Absorptions-banden aufweisen. Zur Entwicklung eines adäquaten Strahlungsthermometers für diesen Zweck wurden im Rahmen dieser Arbeit daher durch Ermittlung von Transmissionsspektren von Kohlenstoffdioxid und Wasserdampf bei hohen Temperaturen und Drücken in einer ei-gens hierfür konstruierten Heißgas-Messzelle zunächst Wellenlängenbereiche identifiziert, in welchen die geplanten Messungen möglich sind. Anschließend wurde der Prototyp eines ent-sprechend konfigurierten Strahlungsthermometers im Zuge des Testlaufes einer vollskaligen Gasturbine erfolgreich erprobt.
Weiterhin wurden im Rahmen dieser Arbeit zwei mögliche Verfahren zur berührungslosen Gastemperaturmessung untersucht. Das erste untersuchte Verfahren setzt ebenfalls auf Strah-lungsthermometrie. Dieses Verfahren sieht vor, aufgrund der Temperaturabhängigkeit des spektralen Transmissionsgrades in den Randbereichen von gesättigten Absorptionsbanden von Gasen aus der in diesen Bereichen transmittierten spektralen Strahldichte auf die Gastempera-tur zu schließen. Im Rahmen dieser Arbeit wurden Voruntersuchungen für dieses Tempera-turmessverfahren durchgeführt. So konnten auf der Grundlage von experimentell ermittelten Transmissionsspektren von Kohlenstoffdioxid bei Drücken zwischen 5 kPa und 600 kPa und Gastemperaturen zwischen Raumtemperatur und 1073 K für das geplante Verfahren nutzbare Wellenlängenintervalle insbesondere im Bereich der Kohlenstoffdioxid-Bande bei 4,26 µm identifiziert werden.
Das zweite im Rahmen dieser Arbeit untersuchte Verfahren zur berührungslosen Gastem-peraturmessung basiert auf der Temperaturabhängigkeit der Wellenlängenposition der Trans-missionsminima der Absorptionsbanden von infrarot-aktiven Gasen. Im Hinblick darauf wur-de dieses Phänomen anhand von experimentell bestimmten hochaufgelösten Transmissions-spektren von Kohlenstoffdioxid überprüft. Weiterhin wurden mögliche Wellenlängenbereiche identifiziert und hinsichtlich ihrer Eignung für das geplante Verfahren charakterisiert. Als am vielversprechendsten erwiesen sich hierbei Teilbanden in den Bereichen um 2,7 µm und um 9,2 µm. Unter Beimischung von Stickstoff mit Partialdrücken von bis zu 390 kPa erwies sich zudem auch die Bande bei 4,26 µm als geeignet.
Die im Rahmen dieser Arbeit experimentell ermittelten Transmissionsspektren konnten dar-über hinaus schließlich durch Vergleich mit entsprechenden HITRAN-Simulationen verifiziert werden.
In der S3-Leitlinie der deutschen Gesellschaft für Angiologie und der Gesellschaft für Gefäßmedizin zur Diagnostik, Therapie und Nachsorge der peripheren arteriellen Verschlusskrankheit (pAVK) werden konservative Maßnahmen zur Behandlung der pAVK im Stadium der Claudicatio intermittens (CI) als Basistherapie empfohlen. In Studien haben sie sich als effektiv und zumindest gleichwertig zu invasiven Maßnahmen erwiesen. Untersuchungen in der Versorgungsrealität abseits großer Zentren fehlen jedoch. Die durchgeführte Studie hat die Umsetzung, Umsetzbarkeit und Effektivität der konservativen Therapie der pAVK im nicht-urbanen Umfeld untersucht.
83 Patienten wurden aus einem Patientenkollektiv von gesamt 180 Patienten am Klinikum Main-Spessart Lohr mit der Diagnose einer pAVK im Stadium der CI zur Erhebung retrospektiver Daten sowie zur Durchführung einer Befragung ausgewählt.
Wir konnten zeigen, dass Empfehlungen aus einer vaskulären Sprechstunde in der Behandlungsrealität oftmals nicht oder nur unvollständig umgesetzt werden. Ein großer Anteil von Patienten mit CI erfährt keine oder nur wenig Besserung ihrer Beschwerden und ersucht z.T. schlussendlich eine Intervention. Bei einem Teil des Patientenkollektivs jedoch führt die Umsetzung der konservativen Therapieempfehlungen zu einer deutlichen Verbesserung der Mobilität und des Schmerzempfindens. Bei diesen Patienten konnte vorerst von einer interventionellen oder operativen Therapie abgesehen werden.
Bei der konservativen Therapie der CI im nicht-urbanen Umfeld ergibt sich eine Diskrepanz zwischen den Empfehlungen der Leitlinien und der tatsächlichen Versorgungsrealität. Eine leitlinien-orientierte Behandlung der CI erfordert eine intensivere ambulante Betreuung, eine bessere Schulung von Patienten und nicht spezialisierten Ärzten, eine adäquate Vergütung sowie eine engere Vernetzung der vorhandenen Strukturen.
Die Verletzungsprävention hat im Profisport deutlich an Bedeutung gewonnen. Die medizinische Eingangsuntersuchung könnte um einen sportartspezifischen Leistungstest erweitert werden, um relevante Leistungs- und Funktionsdefizite zu detektieren, die das Auftreten von Verletzungen begünstigen. In einer prospektiven Kohortenstudie über fünf aufeinanderfolgende Spielzeiten wurde hierzu der Kader einer Basketball-Profimannschaft im Rahmen der medizinischen Eingangsuntersuchung einem modifizierten NBA Combine Test unterzogen. Die in den durchgeführten Korrelationsanalysen ermittelten Ergebnisse zeigten tendenziell ein erhöhtes Verletzungsrisiko bei Athleten mit Defiziten in Agilität und Ausdauer. Außerdem konnten positionsspezifische Einflüsse auf das Verletzungsgeschehen festgestellt werden. Einzelne Verletzungen schienen sich jedoch der Einflussnahme der Athleten weitestgehend zu entziehen. Insgesamt ist die Durchführung eines basketballspezifischen Leistungstests als Erweiterung der medizinischen Eignungsuntersuchung sinnvoll um Funktionsdefizite aufzuzeigen und Verletzungsrisiken zu senken. Außerdem können die erhobenen Basiswerte in der Rehabilitation nach aufgetretener Verletzung zur Bestimmung des individuellen Zeitpunkt eines Return-to-Play/-Competition genutzt werden.
Die Rolle von Connexinen und Gap Junction-vermittelter Kommunikation in pluripotenten Stammzellen sowie der frühen Embryonalentwicklung sind bis heute nicht vollständig aufgeklärt. Mutationen in humanen Connexinen verursachen eine Vielzahl von Krankheiten. Connexin-defiziente iPS Zellen stellen eine gute Basis für die Erforschung der Rolle von Connexinen während der Embryonalentwicklung und bei der Krankheitsentstehung dar.
Das Ziel der vorliegenden Arbeit war es, das CRISPR/Cas9-System in pluripotenten Stammzellen erfolgreich anzuwenden und ein Protokoll zur Erstellung verschiedener Cx43-Defektmutanten zu entwerfen. Nach der Etablierung der CRSIPR/Cas9-Methode in HEK293T-Zellen konnte in der vorliegenden Arbeit darüber hinaus erfolgreich eine Cx43-Defizienz in FSiPS-Zellen erzeugt werden. Weiterhin wurden mehrere Cx43-Mutanten geschaffen und initial auf Pluripotenzmarker und ihr Differenzierungspotential untersucht.
Diese Arbeit bildet die Basis für weitere Untersuchungen des Cx43 in iPS-Zellklonen und davon abgeleiteten Zelltypen sowie artifiziellen 3D-Gewebekulturen. Darüber hinaus bildet sie die Grundlage für die Bildung weiterer Connexin-Defektmutanten sowie von iPS-Zellen mit krankheitsrelevanten Mutationen.
Die Enzyme TNSALP (Tissue Non-Specific Alkaline Phosphatase), ENPP1 (Ectonucleotide Pyrophosphatase/Phosphodiesterase 1) und ANKH (Ankylosis, progressive human homolog) bilden zusammen eine zentrale Regulierungseinheit für den Pyrophosphat (PPi)-Stoffwechsel der Zelle [1, 2].
Störungen dieses genau geregelten Prozesses resultieren in schwerwiegenden Erkrankungen, wie z.B. bei der Hypophosphatasie [3]. Dieser meist autosomal rezessiv vererbten Erkrankung liegt eine durch genetische Mutationen beeinträchtigte Funktion der TNSALP zugrunde, wodurch sich die PPi- Konzentration im Microenvironment der Zelle erhöht. Diese kann im Knochengewebe zu schweren Mineralisierungsstörungen führen [1, 2].
Andere Krankheiten, mit erniedrigten PPi- Konzentrationen, werden mit pathologischen Verkalkungen in verschiedensten Geweben in Verbindung gebracht [4, 5]. Diese gehen unter anderem auf genetische Defekte von ENPP1 zurück[4].
Auch der Mevalonat-Pathway trägt zur Komposition des Microenvironments bezüglich der Homöostase von Phosphaten bei [6, 7]. Hier bestehen auch medizinisch relevante Einflussmöglichkeiten, zum Beispiel durch Bisphosphonate, bei der sogenannten Volkskrankheit Osteoporose.
In dieser Arbeit wurden die Auswirkungen einer PPi-Belastung auf die in vitro Mineralisierung von Mesenchymalen Stammzellen untersucht, wobei Modulatoren der Enzymaktivität für ALP und ENPP1 und der Aktivität des PPi-Kanals ANKH sowie des Mevalonatstoffwechsels zum Einsatz kamen (PPi, Pyridoxalphosphat (PLP), Probenecid, Vitamin D, PPADS (Pyridoxalphosphat-6-azophenyl-2‘,4‘-disulfid Säure) und ß-γmeATP (ß-γ Methylentriphosphat)).
Die Resultate zeigen, dass die Modulation der PPi-Konzentration bei der osteogenen Differenzierung von hMSCs in vitro keine eindeutigen Effekte bewirkt. Geringe Änderungen des Genexpressionsmusters sind letztlich nicht auszuschliessen, blieben jedoch aufgrund der hohen Spendervariabilität durch eine erhöhte Anzahl von Experimenten zu beweisen.
Diese Arbeit zeigt insgesamt eine unerwartet geringe Auswirkung einer exogenen und endogenen Modulation der PPi-Konzentration sowohl mit Blick auf die rein physikalischen Phänomene der Mineralisierung, als auch mit Blick auf die untersuchte Genregulation der wichtigsten beteiligten Proteine, was möglicherweise die hohe Kompensationskapazität der Systeme unter physiologischen Bedingungen reflektiert. Untersuchungen auf proteomischer Ebene, besonders mit Blick auf die Prozessierung von Polypeptiden mit Mineralisierungs-modulierender Wirkung würden möglicherweise genaueren Einblick vermitteln.
Eine genauere Untersuchung der Einflüsse von ENPP1 erscheint für die Zukunft vielversprechend. Allerdings treten hier, besonders auch durch die verwendeten Hemmstoffe der ENPP1, die Phänomene der Vernetzung des Stoffwechsels der Phosphate (inklusive ATP und seiner Metabolite) mit dem Purinergen Signalling deutlich zutage. Diese Vernetzung generiert durch ihre Komplexität sowohl klinisch als auch zellbiologisch/biochemisch erhebliche Interpretationsprobleme, die zukünftige Arbeiten auflösen müssen. Dabei sollte besondere Aufmerksamkeit auf zwei für HPP-PatientInnen klinisch in Zukunft potentiell bedeutsame Ergebnisse gelegt werden, die möglicherweise ungünstigen Auswirkungen einer Therapie mit Probenecid auf die ALPL Expression und die Steigerung der ALPL Expression unter Hemmstoffen des Enzyms ENPP1.

In der vorliegenden Arbeit wurde die Rolle der Transkriptionsfaktoren NFATc1/αA bzw. NFATc1/ßC und NFATc2 in NK-Zellen und die Rolle der Faktoren NFATc1-4 und NFAT5 in Keratinozyten analysiert. Die Familie der Nuclear Factor of Activated T-cell (NFAT) Transkriptionsfaktoren besteht aus fünf Mitgliedern, welche entscheidend die Gentranskription bei Immunantworten beeinflussen. Nach Antigenaktivierung wird in Lymphozyten die Expression des Nfatc1-Gens stark induziert. Die Akkumulation der dabei gebildeten kurzen Isoform NFATc1/αA ist für die Zytokinproduktion als Effektorfunktion sowie für die Proliferation und das Überleben aktivierter Zellen verantwortlich. Das kurze NFATc1-Protein unterscheidet sich nicht nur strukturell, sondern auch funktionell von fast allen anderen NFAT-Proteinen. Um neue Erkenntnisse über die Funktion der Isoform NFATc1/αA in Lymphozyten zu gewinnen, wurden KT12 NK-Zellen mit NFATc1/αA bzw. NFATc1/ßC und NFATc2- exprimierenden Vektoren transfiziert, geklont, stimuliert und anschließend auf ihre jeweilige Apoptoserate und die Zytokinsynthese bzw. -expression hin untersucht. Die gewählten KT12-Hybridoma-Zellen erwiesen sich allerdings in ihrer intendierten Funktion als Testzellen für die Über-Expression von NFATc-Proteinen in NK-Zellen als ungeeignet.
Fehlregulationen von NFAT-Signalwegen werden mit einer fehlerhaften Entwicklung des Immunsystems, mit der Entstehung von Autoimmunerkrankungen und mit Krebs in Verbindung gebracht. Um die Rolle von NFAT-Faktoren in Keratinozyten besser zu verstehen, wurden HaCaT-Zellen und primäre humane Keratinozyten mit Differenzierungssignalen bzw. UVB-Licht stimuliert. Änderungen der Transkription von NFAT-Faktoren, Keratinozyten-spezifischen Proteinen und Chemokinen wurden mittels qRT-PCR-Assays detektiert und analysiert. Insgesamt konnte die Beteiligung von NFAT-Faktoren am Differenzierungsprozess und an der UV-Antwort von Keratinozyten gezeigt werden. Es zeigte sich tendenziell eine stärkere Induktion der kurzen NFATc1-Isoform im Vergleich zu langen NFATc1-Isoformen, was die Frage nach einer besonderen Funktion der kurzen NFATc1-Isoform in Keratinozyten aufwirft. Generell ließen sich besonders hohe Expressionslevel des Transkriptionsfaktors NFAT5 - verglichen mit anderen NFAT-Faktoren - messen. Für die Entwicklung von Therapien, welche die Ursachen und Folgen einer dysregulierten Hautbarrierenbildung behandeln, könnten sich weitere Studien zu einzelnen NFAT-Faktoren bzw. NFATc1-Isoformen als zielführend erweisen.
Fatigue gilt als eine der Häufigsten Nebenerscheinungen einer Krebserkrankung und ihrer Therapie. Das Ziel dieser Queschnittsstudie war es die Häufigkeit und das Unterstützungsbedürfnis von Fatigue bei Krebserkrankten während der Strahlentherapie zu untersuchen und mögliche Ansätze eines Unterstützungsangebotes darzustellen.
Critical size bone defects and nonunion fractures remain difficult to treat. Although cell‐loaded bone substitutes have improved bone ingrowth and formation, the lack of methods for achieving viability and the uniform distribution of cells in the scaffold limits their use as bone grafts. In addition, the predominant mechanical stimulus that drives early osteogenic cell maturation has not been clearly identified. Further, it is challenging to evaluate mechanical stimuli (i.e., deformation and fluid–flow-induced shear stress) because they are interdependent. This thesis compares different mechanical stimuli applied to cell-seeded scaffolds to develop bone grafts efficiently for the treatment of critical size bone defects. It also seeks to understand how deformation strain and interstitial fluid–flow-induced shear stress promote osteogenic lineage commitment. In this thesis, different scaffolds were seeded with primary human bone marrow mesenchymal stem cells (BM-MSCs) from different donors and subjected to static and dynamic culture conditions. In contrast with the static culture conditions, homogenous cell distributions were accomplished under dynamic culture conditions. Additionally, the induction of osteogenic lineage commitment without the addition of soluble factors was observed in the bioreactor system after one week of cell culture. To determine the role of mechanical stimuli, a bioreactor was developed to apply mechanical deformation force to a mesenchymal stem sell (MSC) line (telomerase reverse transcriptase (TERT)) expressing a strain-responsive AP-1 luciferase reporter construct on porous scaffolds. Increased luciferase expression was observed in the deformation strain compared with the shear stress strain. Furthermore, the expression of osteogenic lineage commitment markers such as osteonectin, osteocalcin (OC), osteopontin, runt-related transcription factor 2 (RUNX2), alkaline phosphate (AP), and collagen type 1 was significantly downregulated in the shear stress strain compared with the deformation strain. These findings establish that the deformation strain was the predominant stimulus causing skeletal precursors to undergo osteogenesis in earlier stages of osteogenic cell maturation. Finally, these findings were used to develop a bioreactor in vitro test system in which the effect of medication on osteoporosis could be tested. Primary human BM-MSCs from osteoporotic donors were subjected to strontium ranelate (an osteoporotic drug marketed as Protelos®). Increased expression of collagen type 1 and calcification was seen in the drugtreated osteoporotic stem cells compared with the nondrug-treated osteoporotic stem cells. Thus, this bioreactor technology can easily be adapted into an in vitro osteoporotic drug testing system.
Kohlenstoffmonoxid ist ein wichtiges kleines Signalmolekül das im menschlichen Körper durch die enzymatische Wirkung von Häm-Oxygenase (HO) auf Häm produziert wird. Für eine thera-peutische Anwendung werden Metallcarbonyl-Komplexe als CO-releasing molecules (CORMs) untersucht, die eine kontrollierte Freisetzung in biologischen Zielstrukturen erlauben. Dafür wird entweder die Ligandenperipherie ("drug sphere") modifiziert oder die CORMs an bio-molekulare Trägersysteme konjugiert.
Im Rahmen dieser Arbeit stand dabei die lichtinduzierte Freisetzung von Kohlenstoffmonoxid aus Mangan(I)tricarbonyl-Komplexen im Vordergrund. Die oktaedrische Koordinationssphäre des Metallzentrums wurde dabei durch verschiedene faciale tridentate Liganden komplettiert, welche außerdem eine einfache und modulare Verknüpfung mit biologischen Träger-molekülen ermöglichen sollten. Als Chelatoren wurden Derivate von N,N-Bis(pyridin-2-ylmethyl)amin (bpa) ausgewählt, in denen das zentrale Stickstoffatom mit Alkylaminen unterschiedlicher Kettenlänge funktionalisiert ist, welche über Amid-Bindungen mit Carboxylat-modifizierten Trägermolekülen verknüpft werden können. Diesen bpa-Liganden sollte ein neuartiges Ligandensystem auf der Basis von N-(Phenanthridin-6-ylmethyl)-N-(chinolin-2-ylmethyl)ethan-1,2-diamin (pqen) gegenübergestellt werden, in denen die Phenanthridin-Gruppe interessante photophysikalische und photochemische Eigenschaften erwarten lässt. Die CO-releasing molecules sollten zudem mit den isostrukturellen Rhenium(I)tricarbonyl-Komplexen verglichen werden, die als Marker für die Fluoreszenz-mikroskopie dienen.
Angsterkrankungen gehören zu den am weitesten verbreiteten psychischen Erkrankungen und stellen eine beträchtliche soziale und wirtschaftliche Herausforderung für unsere Gesellschaft dar. Aversive frühe Erfahrungen sind ein bekannter Risikofaktor für die Entwicklung verschiedener psychischer Erkrankungen, insbesondere Angststörungen. Während der frühen Entwicklung findet die Programmierung der Hypothalamus-Hypophysen-Nebennierenrinden- (HHN)-Achse, die die Ausschüttung des Stresshormons Cortisol in Menschen bzw. Corticosteron in Mäusen steuert, statt. Wenn Individuen in dieser kritischen Phase Stress ausgesetzt sind, wird die regelrechte Ausbildung der HHN-Achse gestört, was zu dysregulierten Verhaltensantworten auf Stressreize im späteren Leben führen kann. Das Serotonin (5-HT)-System als eines der ausgedehntesten Neurotransmittersysteme ist an der Vermittlung der Effekte von früher Stressexposition auf angstähnliche Verhaltensweisen beteiligt.
Das Ziel dieser Studie ist es, die Interaktion zwischen genetischer Prädisposition und negativen Einflüssen in frühen Entwicklungsstadien auf die Ausbildung von Angstverhalten im Erwachsenenalter näher zu beleuchten.
In dieser Studie wurden Tryptophanhydroxylase 2 (Tph2)-defiziente weibliche Mäuse als Modell für ein lebenslanges konstitutives 5-HT Synthesedefizit im zentralen Nervensystem verwendet. Nachkommen dieser Mauslinie wurden im frühen Lebensalter Maternaler Separation (MS), d.h. einem mütterlichen Trennungsparadigma, unterzogen und im Erwachsenenalter im „Open field“ (OF) oder in der „Dark-light box“ (DLB) getestet. Im Anschluss an die Verhaltensexperimente wurde die neuronale Aktivierung immunhistochemisch durch Darstellung des frühzeitig auftretenden Genprodukts c-Fos bestimmt.
In der DLB zeigten homozygot Tph2-defiziente Mäuse eine verringerte motorische Aktivität im hellen Kompartiment, und dieser Effekt konnte durch MS normalisiert werden. Zusätzlich verstärkte MS bei diesem Genotyp das Auftreten von fluchtartigen Sprüngen. Im OF hat MS fluchtartige Verhaltensweisen in homo- und heterozygoten Tph2-defizienten Mäusen befördert.
Beide Verhaltenstests führten zu spezifischen neuronalen Aktivierungsmustern, die mithilfe von c-Fos- Immunhistochemie ausgewertet wurden. Die Durchführung des DLB-Tests führte in Abhängigkeit vom Vorhandensein von Tph2 zur Aktivierung des paraventrikulären Kerns des Hypothalamus (PVN) und der basolateralen Amygdala (BL), wohingegen die Exposition gegenüber dem OF-Test zu einer Aktivierung der lateralen Amygdala (La) in Tieren, die einem mütterlichen Trennungsparadigma unterzogen wurden, sowie einer Aktivierung des ventrolateralen (VLPAG) und dorsolateralen (DLPAG) periaquäduktalen Höhlengraus in Abhängigkeit von Tph2 und MS führte.
Zusammenfassend weisen die Ergebnisse dieser Studie darauf hin, dass MS aktive Verhaltensantworten auf aversive Reize in Abhängigkeit vom Vorhandensein von 5-HT im Gehirn fördert. Diese Effekte könnten durch die spezifische Aktivierung von mit Angstverhalten in Zusammenhang stehenden Gehirnregionen während der Verhaltensexperimente vermittelt werden.
Eine ernstzunehmende Nebenwirkung der anti-resorptiven Therapie (AR-Therapie) beim Multiplem Myelom ist die Medikamenten-assoziierten Kiefernekrose. Für die vorliegende Arbeit wurden 50 Patienten mit Medikamenten-assoziierter Kiefernekrose (MRONJ-Gruppe) einer gleich großen Kontrollgruppe ohne Medikamenten- assoziierter Kiefernekrose (KTRL-Gruppe) gegenübergestellt. In der MRONJ-Gruppe dauerte die AR-Therapie signifikant länger als in der KTRL-Gruppe (p < 0,001). Die MRONJ-Patienten erhielten die AR-Therapie im Schnitt knapp 4 Jahre, die KTRL- Patienten 2,5 Jahre. Zudem wurde den MRONJ-Patienten die AR-Therapie signifikant häufiger im 4-wöchentlichen Intervall verabreicht als den KTRL-Patienten (n = 49 vs. n = 36, p = 0,003). Das mediane Gesamtüberleben der MRONJ-Gruppe lag signifikant über dem Gesamtüberleben der KTRL-Gruppe (126 vs. 86 Monate, p = 0,013). Das mediane Gesamtüberleben des gesamten Patientenkollektivs lag bei 111 Monaten. Zudem korrelierte das Gesamtüberleben aller Patienten dieser Arbeit signifikant mit der kumulativen Zoledronatdosis (p < 0,001, r = 0,557). Die Stadieneinteilung und die CRAB-Kriterien zeigten bei Erstdiagnose keine signifikanten Unterschiede zwischen den Gruppen. Die Gründe für das längere Gesamtüberleben der MRONJ-Gruppe können auf die Unterschiede in der AR-Therapie zurückgeführt werden. Es bestand ein signifikanter Unterschied in der Therapiedauer, dem verabreichten Intervall und der kumulativen Zoledronatdosis zwischen den beiden Gruppen. Die Sinnhaftigkeit der Fortführung der AR-Therapie muss regelmäßig evaluiert werden und eine engmaschige Untersuchung des stomatognathen Systems ist von höchster Relevanz, um ein längeres Überleben bei guter Lebensqualität zu ermöglichen.
Alzheimer’s disease (AD) is the most common form of dementia, and currently, there is no treatment to cure or halt disease progression. Because the one-target strategy focusing on amyloid-β has failed to generate successful pharmaceutical treatment, this work studies natural products with pleiotropic effects focusing on oxidative stress and neuroinflammation as key drivers of disease progression. The central part of this work focused on flavonoids as neuroprotectants. 7-O-Esters of taxifolin and cinnamic or ferulic acid were synthesized and investigated towards their neuroprotective potential addressing aging and disease. 7-O-Feruloyl- and 7-O-cinnamoyltaxifolin showed overadditive effects in oxidative stress-induced assays in the mouse neuronal cell line HT22 and proved to be protective against neuroinflammation in microglial BV-2 cells. The overadditive effect translated to animals using an Aβ25-35-induced memory-impaired AD mouse model where the compounds were able to ameliorate short-term memory defects. While the disease-modifying effects in vivo were observed, the detailed mechanisms of action and intracellular targets of the compounds remained unclear. Hence, a chemical probe of the neuroprotective flavonoid ester 7-O-cinnamoyltaxifolin was developed and applied in an activity-based protein profiling approach. SERCA and ANT-1 were identified as potential targets. Further, chemical modifications on the flavonoids taxifolin, quercetin, and fisetin were performed. The achievements of this work are an important contribution to the use of secondary plant metabolites as neuroprotectants. Chemical modifications increased the neuroprotective effect of the natural products, and distinct intracellular pathways involved in the neuroprotective mechanisms were identified. The results of this work support the use of secondary plant metabolites as potential therapeutics and hint towards new pharmacological targets for the treatment of neurodegenerative disorders.
Phonetic and phonological variability in the L1 and L2 of late bilinguals: The case of /r/ and /l/

A large body of research has shown that a late bilingual’s L1 and L2 phonetic categories influence each other, yielding deviations from monolingual norms in the phonetics of both languages. Existing models of L2 sound acquisition (e.g., the Speech Learning Model; Flege, 1995, 2007) predict unified phonetic spaces which accommodate both L1 and L2 sound categories. Such connections between an L1 and an L2 are believed to lead to persistent non-nativelikeness in the L2, but also to divergence from the monolingual norm in the L1, as shown in numerous studies (e.g., Bergmann et al., 2016). In this dissertation, I focus on the differences in the sound patterns of a bilingual’s languages which do not only emerge in the precise phonetic realizations of L1 sounds but also in language-specific distributional patterns that determine the realization of these sound categories in different phonetic contexts. Previous work in L1 attrition is limited to a small set of phonetic properties (especially VOT, e.g., Flege, 1987), variables beyond L2 transfer which are known to give rise to variable realizations have been neglected. Thus, little is known as to whether bilinguals’ realizations of an L1 sound category in different phonetic contexts (e.g., position within a syllable) are subject to change in L1 attrition, and whether such changes arise due to long-term exposure to different distributional patterns of an equivalent L2 category.
In this dissertation I address these gaps by exploring L1 attrition in the distributional and phonetic characteristics of liquids to shed light on the contribution of the L2 and the role of general phonetic and phonological variables to the processes that drive change in an L1. I investigate changes to phonetic properties and distributional patterns of rhoticity and /l/-allophony in the L1 of American-German late bilinguals, a language constellation which offers an instructive test case to investigate the causes of L1 attrition as well as the source from which changes due to L1 attrition emerge. Furthermore, changes to liquids can also shed light on the processes which drive sound change, gradience and variability due to various positional and phonetic factors (e.g., preceding vowel, syllable structure) in liquids across many native varieties of English. In particular, I explore the variable realization and distributional patterns of two sounds known to be subject to a considerable degree of gradience and variability, namely English /r/ and /l/, in American English-German late bilinguals.
To that end, I present the results of a production study of 12 L2-dominant American English-German late bilinguals as well as a monolingual control group for each language. The speakers performed a variety of production tasks which were aimed to elicit the realization of (non)-rhoticity and /l/-(non-)allophony in both languages of the late bilinguals, English and German which were analyzed auditorily (/r/ only) and acoustically (/r/ and /l/). Although L1 attrition of rhotics and laterals has been investigated previously (e.g., de Leeuw, 2008; Ulbrich & Ordin, 2014), the effect of contextual variables on L1 attrition and whether such variables also shape L1 attrition remains unexplored.
The results of the auditory analyses of postvocalic /r/ revealed that the late bilinguals showed non-convergence with monolingual (non-)rhoticity in both of their languages by vocalizing postvocalic /r/ more frequently in their L1 (English) and failing to entirely suppress rhoticity in their L2 (German) leading up to a higher degree of rhoticity in their L2. While the loss of rhoticity in the bilingual’s English was distributed along a spectrum of contextual constraints (e.g., type of pre-rhotic vowel and morpho-phonological environment) known to affect rhoticity in other English varieties, the non-targetlike productions of non-rhoticity (i.e., non-vocalized postvocalic /r/) in their L2, German, were not sensitive to the same contextual constraints. The acoustic analyses of the bilinguals’ rhotic productions in English and German differed from the monolinguals in the acoustic correlates of rhoticity in pre-rhotic vowels where they showed reduced anticipatory F3-lowering (i.e., less /r/-colored vowels).
I take my results to indicate that the bilinguals operate in two separate phonological grammars which approximate the respective L1 norm but show an increase of variability along constraints already present in each grammar. In contrast, the bilinguals’ phonetic system seem shared between the two grammars. This leads to persistent L1-L2-interactions as the two grammars operate within the same phonetic space. Thus, the changes in L1 attrition are induced but not governed by the L2: Change to the L1 reflects constraints underlying the L1 as well as more general laws of phonetics and universal trajectories of language change.
The lateral results revealed that just like in postvocalic /r/, the bilinguals showed non-convergence with the monolingual norm regarding the velarization of coda /l/ in both their languages. The changes to English laterals were sensitive to their positional context and more substantial for word-initial laterals than word-final laterals. Similarly, their German laterals were non-convergent with the monolinguals in two ways. Firstly, the bilinguals differed with regard to the acoustic specifications of their laterals, and secondly, the bilinguals failed to suppress the lateral allophony from their L1, leading to a non-targetlike allophonic pattern in their L2 laterals.
I interpret the lateral results to lack evidence that the L1 allophonic rule was affected by the presence of an L2; nevertheless, L1 change emerged in the phonetic specifications of laterals. Furthermore, the bilinguals did not establish a nativelike allophonic pattern in their L2, leading to non-convergence in the allophonic distribution as well as the phonetic realization of German laterals.
In this way, this dissertation provides evidence for L1 attrition in the distributional and the phonetic properties of liquids in the L1 of late bilinguals. In particular, the study presented in this dissertation provides evidence that L1 attrition is induced by the presence of a similar sound pattern in the L2. The pathway of attrition follows constraints not only underlyingly present in the L1 but also part of the universal laws of phonetics known to shape sound change. To explain these results, I draw from existing constraint grammars in phonological theory (such as Optimality Theory and Harmonic Grammar) to develop my Dynamic Constraints approach which allows the effects of external variables (e.g., L2 acquisition and its effect on the mind), and internal variables such as an increased likelihood of variability due to articulatory differences can be modeled using scaling factors which can interact with each other, the noise within the grammars, and the constraint weight itself. In this way, the model links previous findings on L1 attrition and its connections to diachronic and synchronic variability, offering insights into the links between the individual languages in a bilingual’s mind.
In einer vorangegangenen Phase-1-Studie wurde beobachtet, dass bei Patienten mit schwerer aneurysmatischer SAB, die CBF durch intermittierende kontrollierte Hyperkapnie erhöht werden kann. Zudem zeigte sich in dieser vorherigen Studie nach dem Zurücksetzen der mechanischen Beatmung auf die Ausgangsparameter eine langsame und asymptotische Rückkehr der CBF zu den Ausgangsniveaus ohne einen negativen Rebound-Effekt. Diese Beobachtung legte nahe, dass eine längere Dauer der Hyperkapnie den CBF-erhöhenden Effekt verlängern kann.
Die vorliegende Studie wurde als Dosisoptimierungsstudie geplant, um den Zeitpunkt zu bestimmen, zu dem der CBF ein Maximum erreicht, und unter der Annahme, dass nach diesem Maximum Puffermechanismen in Blut und Liquor zu Anpassungsmechanismen führen können, die nach dem Abbruch zu einem negativen Rebound-Effekt führen.
Es ergab sich eine "Netto" - Optimaldauer der Hyperkapnieintervention von 45 Minuten.
Hintergrund: Geflüchtete haben ein hohes Risiko, Multiresistente Erreger (MRE) zu tragen. Infektionen mit MRE (Multiresistente Gram-negative Bakterien [MRGN] und Methicillin-resistenter Staphylococcus aureus [MRSA]) sind mit einer erhöhten Mortalität, Krankenhausaufenthaltsdauer und Krankenhauskosten assoziiert. Der Einfluss von prädisponierenden Faktoren für eine Besiedlung mit MRE ist für Geflüchtete noch unzureichend erforscht. Kenntnisse über prädisponierende Faktoren können helfen, Infektionsschutzmaßnahmen für Geflüchtete in Krankenhäusern anzupassen.
Methodik: Von November 2015 bis April 2016 wurden 134 Geflüchtete am Universitätsklinikum Würzburg auf MRE im Nasen-/Rachen- (MRSA), Rektal- (MDRGN-Enterobacteriaceae, MDRGN-Pseudomonas aeruginosa) und Haut-/Rachenabstrich (MDRGN-Acinetobacter baumannii) gescreent.
Ergebnisse: 62,7% von 134 gescreenten Flüchtlingen waren männlichen Geschlechts und das Durchschnittsalter lag bei 19 Jahren [IQR: 7–31]. 23,9% (n=32) zeigten einen positiven MRE-Befund (MRSA: 3,4 % von 118, 2MDRGN-Neopäd: 19,3 % von 57, 3MDRGN: 13,6 % von 125, 4MDRGN: 0 % von 125). Es wurden 25 Escherichia coli (98,3%), 3 Klebsiella pneumoniae (10,7%) und keine positiven Befunde auf Pseudomonas aeruginosa oder Acinetobacter baumannii gefunden. 3 Geflüchtete (9,6%) zeigten eine Mehrfachbesiedlung und 2 Geflüchtete (6,2%) wiesen eine durch MRE bedingte Infektionserkrankung auf (submandibulärer Abszess, Pyelonephritis). Bei 94 Geflüchteten mit vollständigem Screening waren Geflüchtete mit positivem MRE-Befund im Vergleich zu Geflüchteten mit negativem MRE-Befund jüngeren Alters (Medianalter: 8 Jahre [IQR: 3–36] vs. 24 Jahre [IQR: 14–33]) und vermehrt weiblichen Geschlechts (61,1%). Geflüchtete mit positivem MRE-Befund wiesen zudem im Vergleich vermehrt prädisponierende Faktoren auf, bspw. einen vorherigen Krankenhausaufenthalt (61,1 % vs. 35,5%), chronische Pflegebedürftigkeit (16,7 % vs. 1,3 %) oder eine Fluchtanamnese ≤ 3 Monate (80,0 % vs. 29,4 %).
Schlussfolgerung: Prädisponierende Faktoren spielen eine große Rolle für eine Besiedlung mit MRE. Prospektive Studien sollten folgen, um prädisponierende Faktoren für eine Besiedlung mit MRE bei Geflüchteten besser charakterisieren zu können.
Neue physikalische Erkenntnisse vervollständigen die Sicht auf die Welt und erschließen gleichzeitig Wege für Folgeexperimente und technische Anwendungen. Das letzte Jahrzehnt der Festkörperforschung war vom zunehmenden Fokus der theoretischen und experimentellen Erkundung topologischer Materialien geprägt. Eine fundamentale Eigenschaft ist ihre Resistenz gegenüber solchen Störungen, welche spezielle physikalische Symmetrien nicht verletzen. Insbesondere die Topologischen Isolatoren - Halbleiter mit isolierenden Volumen- sowie gleichzeitig leitenden und spinpolarisierten Oberflächenzuständen - sind vielversprechende Kandidaten zur Realisierung breitgefächerter spintronischer Einsatzgebiete. Bis zur Verwirklichung von Quantencomputern und anderer, heute noch exotisch anmutender Konzepte bedarf es allerdings ein umfassenderes Verständnis der grundlegenden, physikalischen Zusammenhänge. Diese kommen vor allem an Grenzflächen zum Tragen, weshalb oberflächensensitive Methoden bei der Entdeckung der Topologischen Isolatoren eine wichtige Rolle spielten.
Im Rahmen dieser Arbeit werden daher strukturelle, elektronische und magnetische Eigenschaften Topologischer Isolatoren mittels Tieftemperatur-Rastertunnelmikroskopie und -spektroskopie sowie begleitenden Methoden untersucht.
Die Veränderung der Element-Ausgangskonzentration während dem Wachstum des prototypischen Topologischen Isolators Bi2Te3 führt zur Realisierung eines topologischen p-n Übergangs innerhalb des Kristalls. Bei einem spezifischen Verhältnis von Bi zu Te in der Schmelze kommt es aufgrund unterschiedlicher Erstarrungstemperaturen der Komponenten zu einer Ansammlung von Bi- und Te-reichen Gegenden an den gegenüberliegenden Enden des Kristalls. In diesen bildet sich infolge des jeweiligen Elementüberschusses durch Kristallersetzungen und -fehlstellen eine Dotierung des Materials aus. Daraus resultiert die Existenz eines Übergangsbereiches, welcher durch Transportmessungen verifiziert werden kann. Mit der räumlich auflösenden Rastertunnelmikroskopie wird diese Gegend lokalisiert und strukturell sowie elektronisch untersucht. Innerhalb des Übergangsbereiches treten charakteristische Kristalldefekte beider Arten auf - eine Defektunterdrückung bleibt folglich aus. Dennoch ist dort der Beitrag der Defekte zum Stromtransport aufgrund ihres gegensätzlichen Dotiercharakters vernachlässigbar, sodass der topologische Oberflächenzustand die maßgeblichen physikalischen Eigenschaften bestimmt. Darüber hinaus tritt der Übergangsbereich in energetischen und räumlichen Größenordnungen auf, die Anwendungen bei Raumtemperatur denkbar machen.
Neben der Veränderung Topologischer Isolatoren durch den gezielten Einsatz intrinsischer Kristalldefekte bieten magnetische Störungen die Möglichkeit zur Prüfung des topologischen Oberflächenzustandes auf dessen Widerstandsfähigkeit sowie der gegenseitigen Wechselwirkungen. Die Zeitumkehrinvarianz ist ursächlich für den topologischen Schutz des Oberflächenzustandes, weshalb magnetische Oberflächen- und Volumendotierung diese Symmetrie brechen und zu neuartigem Verhalten führen kann.
Die Oberflächendotierung Topologischer Isolatoren kann zu einer starken Bandverbiegung und einer energetischen Verschiebung des Fermi-Niveaus führen. Bei einer wohldosierten Menge der Adatome auf p-dotiertem Bi2Te3 kommt die Fermi-Energie innerhalb der Volumenzustands-Bandlücke zum Liegen. Folglich wird bei Energien rund um das Fermi-Niveau lediglich der topologische Oberflächenzustand bevölkert, welcher eine Wechselwirkung zwischen den Adatomen vermitteln kann. Für Mn-Adatome kann Rückstreuung beobachtet werden, die aufgrund der Zeitumkehrinvarianz in undotierten Topologischen Isolatoren verboten ist. Die überraschenderweise starken und fokussierten Streuintensitäten über mesoskopische Distanzen hinweg resultieren aus der ferromagnetischen Kopplung nahegelegener Adsorbate, was durch theoretische Berechnungen und Röntgendichroismus-Untersuchungen bestätigt wird. Gleichwohl wird für die Proben ein superparamagnetisches Verhalten beobachtet.
Im Gegensatz dazu führt die ausreichende Volumendotierung von Sb2Te3 mit V-Atomen zu einem weitreichend ferromagnetischen Verhalten. Erstaunlicherweise kann trotz der weitläufig verbreiteten Theorie Zeitumkehrinvarianz-gebrochener Dirac-Zustände und der experimentellen Entdeckung des Anormalen Quanten-Hall-Effektes in ähnlichen Probensystemen keinerlei Anzeichen einer spektroskopischen Bandlücke beobachtet werden. Dies ist eine direkte Auswirkung der dualen Natur der magnetischen Adatome: Während sie einerseits eine magnetisch induzierte Bandlücke öffnen, besetzen sie diese durch Störstellenresonanzen wieder. Ihr stark lokaler Charakter kann durch die Aufnahme ihrer räumlichen Verteilung aufgezeichnet werden und führt zu einer Mobilitäts-Bandlücke, deren Indizien durch vergleichende Untersuchungen an undotiertem und dotiertem Sb2Te3 bestätigt werden.
Die tiefe Hirnstimulation ist eine etablierte und hocheffiziente operative Behandlungsmethode für Patienten mit idiopathischem Parkinson- Syndrom (IPS). Als Zielgebiet dient in den meisten Fällen der Nucleus subthalamicus. Die Indikationen zur Implantation einer tiefen Hirnstimulation (THS) sind medikamentös nicht behandelbare motorische Fluktuationen und Dyskinesien oder ein medikamentös nicht kontrollierbarer Tremor.
Bislang erfolgt eine kontinuierliche Stimulation. Little et al. konnten jedoch bereits in ihrer 2013 veröffentlichen Studie zeigen, dass eine adaptive Stimulation, gemessen am UPDRS, um 27 % effektiver war und entsprechend die Stimulationszeit um 56 % gesenkt werden konnte.
Voraussetzung für die Anwendbarkeit einer adaptiven Stimulation im klinischen Alltag ist der Nachweis eines oder mehrerer Physiomarker, welche als Rückkopplungssignal für den Stimulationsbeginn dienen. Diese Marker müssen verlässlich mit dem Auftreten und der Ausprägung der Bewegungsstörungen korrelieren. Die Systeme müssen die Signale auslesen und entsprechend darauf reagieren können, damit ein sogenanntes Closed- loop- Verfahren entstehen kann. Bei diesen Markern handelt es sich um sogenannte lokale Feldpotenzialaktivitäten, das heißt niederfrequente Potentialänderungen von Zellen in subkortikalen Arealen des Gehirns, welche über Elektroden der THS abgeleitet werden können. Der Stimulator Activa PC+S (Medtronic) ermöglicht es erstmalig Aufzeichnungen von LFP- Daten, außerhalb eines experimentellen Laboraufbaus, mittels dauerhaft implantiertem Gerät vorzunehmen und damit auch Langzeitanalysen durchzuführen.
Erkenntnisse vergangener Studien ergaben, dass die synchronisierte, pathologisch gesteigerte oszillatorische Aktivität im Beta-Frequenzband (13- 35 Hz) eine bedeutende Rolle im Bezug auf die Pathophysiologie des IPS spielt und als krankheitsspezifische Aktivität gilt. Es konnte bereits belegt werden, dass die Verbesserung der motorischen Symptome (Bradykinese und Rigor) mit dem Ausmaß der Suppression der Betaband- Aktivität korreliert. Die Betabandaktivität als lokale Feldpotentialaktivität kann als Physiomarker einer adaptiven Stimulation dienen.
Unser Hauptaugenmerk galt daher der Analyse der Betabandaktivität oder anderer Frequenzbereiche während des Schlafes um hier die THS bedarfsgerecht einzusetzen. Hierfür wurden nächtliche subkortikale LFP- Aufzeichnungen parallel zur Schlaf- Polysomnographie durchgeführt. Zudem erfolgte in der vorliegenden Arbeit sowohl in unserem Vorversuch als auch in unserem Hauptversuch die Anwendung des UPDRS Teil III zur Erfassung der motorischen Symptome, sowie die Durchführung von Fragebögen zur Erfassung der nicht- motorischen Symptome, insbesondere des Schlafes vor und nach Implantation der tiefen Hirnstimulation.
Wir konnten belegen, dass es nach Implantation der THS zu einer Erhöhung der Schlafeffizienz und zu einer Erhöhung des Anteils der Schlafstadien II und III und damit einhergehend zu einer Steigerung der Schlafqualität kommt. Übereinstimmend mit anderen Studien konnten wir zeigen, dass sich die Motorik unter Stimulation deutlich verbessert. Im Vorversuch reduzierte sich der mittlere präoperative MDS- UPDRS III im MedsOFF verglichen mit dem mittleren postoperativ MDS- UPDRS III im MedsOFF/StimON um 37 %. In der PC+S- Studie imponierte eine Reduktion um 67%. Zudem zeigte sich eine Reduktion der nicht- motorischen Symptome durch die THS, insbesondere in der Kategorie Schlaf. Die Ergebnisse der vorliegenden Arbeit ergaben außerdem, dass die Betabandaktivität im Schlafstadium II und vor allem im Schlafstadium III am geringsten ist. Im Schlafstadium I und REM ist die Betabandaktivität höher als im Schlafstadium II und III. Hierbei war entscheidend, dass die Patienten eine klar abgrenzbare Betabandaktivität im Wachstadium aufwiesen und die Elektrodenkontakte im dorsolateralen Kerngebiet des STN lokalisiert waren.
Gegenläufig dazu verhält sich die Deltaaktivität. Sie ist im Schlafstadium II und besonders im Stadium III am höchsten. Stadium I ist mit durchschnittlich um 7,3 % niedriger als im Wachstadium. Am geringsten ist sie jedoch im REM-Schlafstadium.
Indem wir mit der Betabandaktivität und Deltaaktivität in den einzelnen Schlafstadien einen stabilen und reproduzierbaren Physiomarker finden konnten, sind wir unserem Ziel der adaptiven THS ein Stück näher gekommen.
Das Ziel der vorliegenden Arbeit war den Einfluss des Gewichts und des Rauchens auf die Pharmakokinetik der Psychopharmaka zu zeigen.
Analysiert wurden Antidepressiva Amitriptylin, Doxepin, Es-Citalopram, Mirtazapin und Venlafaxin sowie Antipsychotika Clozapin, Quetiapin und Risperidon. Zur Erhebung der Daten wurden insgesamt 5999 TDM- Anforderungsscheine herangezogen, die in den Jahren 2009 - 2010 im Speziallabor für TDM in der Klinik und Poliklinik für Psychiatrie und Psychotherapie des Universitäsklinikums Wüzburg ausgewertet wurden.
Ein signifikanter Einfluss von Rauchen konnte bei den Serumspiegeln von Amitriptylin, Doxepin, Mirtazapin, Venlafaxin und Clozapin festgestellt werden. Nichtraucher wiesen jeweils signifikant höhere dosiskorrigierte Serumkonzentrationen als Raucher auf.
Diese Ergebnisse liefern somit Hinweise auf mögliche Induktion der Enzyme CYP2C19, CYP1A2 und CYP3A4 durch Tabakrauch.
Bei der Analyse des Einflusses des Körpergewichts auf die Pharmakokinetik konnten signifikante Ergebnisse bei den Substanzen Amitriptylin, Doxepin, Mirtazapin und Venlafaxin gezeigt werden. Bei diesen Substanzen konnten wir niedrigere Serumspiegel mit zunehmenden Gewicht feststellen.
Für diese Ergebnisse könnten zum einen die lipophilen Eigenschaften mancher Psychopharmaka (Nortriptylin, Doxepin) zuständig sein. Zum anderen hat das zunehmende Körpergewicht einen Einfluss auf den Metabolismus der Cytochrom-P450-Enzyme. Somit könnte die mögliche Induktion von CYP2D6, CYP2C19 und CYP3A4 bei Patienten mit höherem Körpergewicht für wirksam niedrigere Serumspiegel der Substanzen bzw. deren Metaboliten verantwortlich sein.
Untersuchung der Begrenzung künstlicher Ernährung am Lebensende auf der Palliativstation der Universitätsklinik Würzburg
Hintergrund: Leitlinien empfehlen die Beendigung lebensverlängernder Interventionen am Lebensende. Wir untersuchten die Relevanz von künstlicher Ernährung und Flüssigkeitszufuhr am Lebensende in einer spezialisierten Palliativstation (SPS) und die alltägliche Praxis einschränkender Entscheidungen. Methoden: Retrospektive Analysen der Akten der verstorbenen Patienten in den Jahren 2012-2014.
Ergebnisse: 397/887 Patienten starben auf der Palliativstation (44,7%). 65/397 Patienten erhielten in den letzten 11 Tagen ihres Lebens (16,3%) entweder künstliche Ernährung (KE) oder therapeutische Flüssigkeitszufuhr (>1000 ml Flüssigkeit, FS). Bei 53/65 Patienten wurde die KE/FS mehr als 48 Stunden vor dem Tod (81,5%) und bei 8/65 Patienten kürzer als 48 Stunden vor dem Tod (12,3%) beendet. 2/65 Patienten erhielten KE und FS bis zum Tod (3,0%). Die Entscheidungsfindung bezüglich der Begrenzung von KE bzw. FS wurde in 44/65 Patientenakten (67,6%) dokumentiert. Die Entscheidungen wurden 2-4 Tage vor dem Tod bei 25/44 Patienten (56,8%), kürzer als 2 Tage vor dem Tod bei 4/44 Patienten (9,0%) und länger als 4 Tage vor dem Tod bei 15/44 Patienten (34,0%) getroffe. Als Gründe wurden angegeben: Beginn der Sterbephase (33/44, 75,0 %), Patientenwunsch (6/44, 13,6%), Nebenwirkungen (3/44, 6,8%) und andere (2/44, 4,5%) ). Bei 43/63 Patienten wurden KE und FS auf einmal beendet (68,2%) und bei 20/63 Patienten wurde es langsam über einen Zeitraum von etwa 48 Stunden beendet (31,7%). 60/65 Patienten erhielten in den letzten 11 Lebenstagen auch potenziell lebensverlängernde Medikamente (60/65, 92,3%). Bei 37/60 Patienten wurde die potenziell lebensverlängernde Medikation (LM) gleichzeitig mit der KE (61,6%) beendet, bei 21/60 Patienten wurde die LM innerhalb von 48 Stunden nach Beendigung der KE (35,0%) beendet und 2 / 65 Patienten erhielten LM bis zum Tod (3,0%), einer von ihnen zusammen mit KE. Die beiden Patienten, die KE und FS bis zum Tod erhielten, blieben kürzer als 48 Stunden auf der Palliativstation. Schlussfolgerung: Die Beendigung von KE und FS war ein relevantes Thema. Ebenso die Beendigung einer potentiell lebensverlängernden Medikation bei diesen Patienten. Auch in einem SPS-Setting ist die Sterbephase nicht leicht zu erkennen und die Entscheidungsfindung scheint Zeit zu brauchen. Ein rigoristischer Ansatz scheint nicht hilfreich zu sein.
Das Ziel der vorliegenden Arbeit war die Untersuchung einer Kohorte erwachsener Patienten mit einseitiger Taubheit (single-sided deafness SSD), die ein Cochlea-Implantat erhalten haben. Es sollten Faktoren ermittelt werden, die es ermöglichen, eine Voraussage über die spätere Hörleistung mit Cochlea Implantat treffen zu können.
Der Beobachtungszeitraum umfasste die Jahre 2009 - 2016. Das Patientenkollektiv bestand aus 55 einseitig ertaubten Erwachsenen. Die Sprachverständlichkeit wurde mit dem Freiburger Zahlentest bei 65 dB SPL, dem Freiburger Einsilbertest bei 65 dB SPL und 80 dB SPL, sowie dem HSM-Satztest ohne Störgeräusch bei 65 dB SPL getestet. Es hat sich gezeigt, dass mit steigender Hörerfahrung mit einer Verbesserung der Hörkompetenz zu rechnen ist. Im untersuchten Patientenkollektiv schnitten Patienten mit der Kombination aus einer langen Ertaubungsdauer und einer entzündlichen Ätiologie signifikant schlechter ab als die Vergleichsgruppe.
The main goal of this thesis is to elucidate the sense in which recent experimental progress in condensed matter physics, namely the verification of two-dimensional Dirac-like materials and their control in ballistic- as well as hydrodynamic transport experiments enables the observation of a well-known 'high-energy' phenomenon: The parity anomaly of planar quantum electrodynamics (QED\(_{2+1}\)). In a nutshell, the low-energy physics of two-dimensional Quantum Anomalous Hall (QAH) insulators like (Hg,Mn)Te quantum wells or magnetically doped (Bi,Sb)Te thin films can be described by the combined response of two 2+1 space-time dimensional Chern insulators with a linear dispersion in momentum. Due to their Dirac-like spectra, each of those Chern insulators is directly related to the parity anomaly of planar quantum electrodynamics. However, in contrast to a pure QED\(_{2+1}\) system, the Lagrangian of each Chern insulator is described by two different mass terms: A conventional momentum-independent Dirac mass \(m\), as well as a momentum-dependent so-called Newtonian mass term \(B \vert \mathbf{k} \vert^2\). According to the parity anomaly it is not possible to well-define a parity- and U(1) gauge invariant quantum system in 2+1 space-time dimensions. More precisely, starting with a parity symmetric theory at the classical level, insisting on gauge-invariance at the quantum level necessarily induces parity-odd terms in the calculation of the quantum effective action. The role of the Dirac mass term in the calculation of the effective QED\(_{2+1}\) action has been initially studied in Phys. Rev. Lett. 51, 2077 (1983). Even in the presence of a Dirac mass, the associated fermion determinant diverges and lacks gauge invariance. This requires a proper regularization/renormalizaiton scheme and, as such, transfers the peculiarities of the parity anomaly to the massive case.
In the scope of this thesis, we connect the momentum-dependent Newtonian mass term of a Chern insulator to the parity anomaly. In particular, we reveal, that in the calculation of the effective action, before renormalization, the Newtonian mass term acts similarly to a parity-breaking element of a high-energy regularization scheme. This calculation allows us to derive the finite frequency correction to the DC Hall conductivity of a QAH insulator. We derive that the leading order AC correction contains a term proportional to the Chern number. This term originates from the Newtonian mass and can be measured via electrical or via magneto-optical experiments. The Newtonian mass, in particular, significantly changes the resonance structure of the AC Hall conductivity in comparison to pure Dirac systems like graphene.
In addition, we study the effective action of the aforementioned Chern insulators in external out-of-plane magnetic fields. We show that as a consequence of the parity anomaly the QAH phase in (Hg,Mn)Te quantum wells or in magnetically doped (Bi,Sb)Te thin films survives in out-of-plane magnetic fields, violates the Onsager relation, and can therefore be distinguished from a conventional quantum Hall (QH) response. As a smoking-gun of the QAH phase in increasing magnetic fields, we predict a transition from a quantized Hall plateau with \(\sigma_\mathrm{xy}= -\mathrm{e}^2/\mathrm{h}\) to a not perfectly quantized plateau which is caused by scattering processes between counter-propagating QH and QAH edge states. This transition is expected to be of significant relevance in paramagnetic QAH insulators like (Hg,Mn)Te/CdTe quantum wells, in which the exchange interaction competes against the out-of-plane magnetic field.
All of the aforementioned results do not incorporate finite temperature effects. In order to shed light on such phenomena, we further analyze the finite temperature Hall response of 2+1 dimensional Chern insulators under the combined influence of a chemical potential and an out-of-plane magnetic field. As we have mentioned above, this non-dissipative transport coefficient is directly related to the parity anomaly of planar quantum electrodynamics. Within the scope of our analysis we show that the parity anomaly itself is not renormalized by finite temperature effects. However, the parity anomaly induces two terms of different physical origin in the effective Chern-Simons action of a QAH insulator, which are directly proportional to its Hall conductivity. The first term is temperature and chemical potential independent and solely encodes the intrinsic topological response. The second term specifies the non-topological thermal response of conduction- and valence band modes, respectively. We show that the relativistic mass \(m\) of a Chern insulator counteracts finite temperature effects, whereas its non-relativistic Newtonian mass \(B \vert \mathbf{k} \vert^2 \) enhances these corrections. In addition, we are extending our associated analysis to finite out-of-plane magnetic fields, and relate the thermal response of a Chern insulator therein to the spectral asymmetry, which is a measure of the parity anomaly in out-of-plane magnetic fields.
In the second part of this thesis, we study the hydrodynamic properties of two-dimensional electron systems with a broken time-reversal and parity symmetry. Within this analysis we are mainly focusing on the non-dissipative transport features originating from a peculiar hydrodynamic transport coefficient: The Hall viscosity \(\eta_\mathrm{H}\). In out-of-plane magnetic fields, the Hall viscous force directly competes with the Lorentz force, as both mechanisms contribute to the overall Hall voltage. In our theoretical considerations, we present a way of uniquely distinguishing these two contributions in a two-dimensional channel geometry by calculating their functional dependencies on all external parameters. We are in particular deriving that the ratio of the Hall viscous contribution to the Lorentz force contribution is negative and that its absolute value decreases with an increasing width, slip-length and carrier density. Instead, it increases with the electron-electron mean free path in the channel geometry considered. We show that in typical materials such as GaAs the Hall viscous contribution can dominate the Lorentz signal up to a few tens of millitesla until the total Hall voltage vanishes and eventually is exceeded by the Lorentz contribution. Last but not least, we derive that the total Hall electric field has a parabolic form originating from Lorentz effects. Most remarkably, the offset of this parabola is directly characterized by the Hall viscosity. Therefore, in summary, our results pave the way to measure and to identify the Hall viscosity via both global and local measurements of the entire Hall voltage.
Auch wenn die Ätiopathogenese von Morbus Parkinson bis heute nicht vollständig geklärt ist, scheint α-Synuclein (α-Syn) eine zentrale Rolle zu spielen. Die Entdeckung als genetische Ursache der Erkrankung, als Hauptbestandteil der Lewy-Körper (LK) und seine Assoziation mit verschiedenen anderen potenziellen ätiologischen Faktoren verdeutlichen dies.
Bei Ratten und Affen führte eine AAV1/2-vermittelte Überexpression von A53T-α-Syn zu einer Degeneration dopaminerger Neurone in der Substantia nigra (SN), einem striatalen dopaminergen Defizit sowie Verhaltensauffälligkeiten. In Anbetracht bestimmter Vorteile der Mausspezies, war es das Ziel dieser Dissertation - die im Rahmen eines kollaborativen Projektes mit dem Toronto Western Research Institut in Ontario, Kanada entstanden ist - dieses auf AAV1/2-A53T-α-Syn basierende Parkinson-Modell auf Mäuse zu übertragen.
Dazu wurde AAV1/2-A53T-α-Syn oder leerer AAV1/2-Vektor in einer Dosis von 1,5 µl mit einer Konzentration von 5,16 x 10^12 gp/ml stereotaktisch einseitig in die rechte SN von C57BL/6-wt-Mäusen injiziert. Über einen Zeitraum von 11 Wochen wurden verschiedene Verhaltensexperimente durchgeführt und die beiden Versuchstiergruppen miteinander verglichen. Post-mortem erfolgten verschiedene immunhistochemische Untersuchungen.
Es konnte gezeigt werden, dass die einseitige Injektion von AAV1/2-A53T-α-Syn in die SN bei Mäusen eine weit verbreitete Überexpression von A53T-α-Syn in dopaminergen Neuronen der SN induzierte, die innerhalb von 10 Wochen zu signifikanten frühen und persistierenden motorischen Verhaltensauffälligkeiten, nigrostriataler Degeneration und Entwicklung einer Lewy-ähnlichen Pathologie führte.
Durch die Generierung und Charakterisierung dieses neuen Parkinson-Mausmodells, das klinische und histopathologische Merkmale der menschlichen Erkrankung widerspiegelt, besteht nun die Möglichkeit es weiterzuentwickeln und z.B. auf transgene Mäuse zu übertragen, um u.a. molekulare Mechanismen der Parkinson-Krankheit zu entschlüsseln und präklinische Tests von krankheitsmodifizierenden Therapien durchzuführen.
Pointing is a ubiquitous means of communication. Nevertheless, observers systematically misinterpret the location indicated by pointers. We examined whether these misunderstandings result from the typically different viewpoints of pointers and observers. Participants either pointed themselves or interpreted points while assuming the pointer’s or a typical observer perspective in a virtual reality environment. The perspective had a strong effect on the relationship between pointing gestures and referents, whereas the task had only a minor influence. This suggests that misunderstandings between pointers and observers primarily result from their typically different viewpoints.
The EC Directive 2013/59/Euratom states in article 56 that exposures of target volumes in nuclear medicine treatments shall be individually planned and their delivery appropriately verified. The Directive also mentions that medical physics experts should always be appropriately involved in those treatments. Although it is obvious that, in nuclear medicine practice, every nuclear medicine physician and physicist should follow national rules and legislation, the EANM considered it necessary to provide guidance on how to interpret the Directive statements for nuclear medicine treatments.
For this purpose, the EANM proposes to distinguish three levels in compliance to the optimization principle in the directive, inspired by the indication of levels in prescribing, recording and reporting of absorbed doses after radiotherapy defined by the International Commission on Radiation Units and Measurements (ICRU):
Most nuclear medicine treatments currently applied in Europe are standardized. The minimum requirement for those treatments is ICRU level 1 (“activity-based prescription and patient-averaged dosimetry”), which is defined by administering the activity within 10% of the intended activity, typically according to the package insert or to the respective EANM guidelines, followed by verification of the therapy delivery, if applicable.
Non-standardized treatments are essentially those in developmental phase or approved radiopharmaceuticals being used off-label with significantly (> 25% more than in the label) higher activities. These treatments should comply with ICRU level 2 (“activity-based prescription and patient-specific dosimetry”), which implies recording and reporting of the absorbed dose to organs at risk and optionally the absorbed dose to treatment regions.
The EANM strongly encourages to foster research that eventually leads to treatment planning according to ICRU level 3 (“dosimetry-guided patient-specific prescription and verification”), whenever possible and relevant.
Evidence for superiority of therapy prescription on basis of patient-specific dosimetry has not been obtained. However, the authors believe that a better understanding of therapy dosimetry, i.e. how much and where the energy is delivered, and radiobiology, i.e. radiation-related processes in tissues, are keys to the long-term improvement of our treatments.
Purpose
In cancer of unknown primary (CUP), positron emission tomography/computed tomography (PET/CT) with the glucose analog [\(^{18}\)F]FDG represents the standard imaging approach for localization of the malignant primary. Frequently, however, [\(^{18}\)F]FDG PET/CT cannot precisely distinguish between small occult tumors and chronic inflammation, especially in Waldeyer’s tonsillar ring. To improve the accuracy for detecting primary tumors in the Waldeyer’s tonsillar ring, the novel PET tracer [\(^{68}\)Ga]Ga-FAPI-4 for specific imaging of fibroblast activation protein (FAP) expression was used as a more specific target for cancer imaging.
Methods
Eight patients with suspicion of a malignant tumor in Waldeyer’s tonsillar ring or a CUP syndrome were examined. PET/CT scans with [\(^{18}\)F]-FDG and [\(^{68}\)Ga]Ga-FAPI-4 were performed for pre-operative tumor localization. After surgical resection, histopathological and immunohistochemical results were compared to PET/CT findings.
Results
Histopathology revealed a palatine or lingual tonsil carcinoma in all patients. In case of lymph node metastases smaller than 7 mm in size, the [\(^{18}\)F]FDG PET/CT detection rate of cervical lymph node metastases was higher than that of [\(^{68}\)Ga]FAPI PET/CT, while both tracers identified the primary tumors in all eight cases. The size of the primary and the lymph node metastases was directly correlated to the respective FAP expression, as detected by immunohistochemistry. The mean SUVmax for the primary tumors was 21.29 ± 7.97 for \(^{18}\)F-FDG and 16.06 ± 6.29 for \(^{68}\)Ga-FAPI, respectively (p = 0.2). The mean SUVmax for the healthy contralateral tonsils was 8.38 ± 2.45 for [\(^{18}\)F]FDG and 3.55 ± 0.47 for [\(^{68}\)Ga]FAPI (p < 0.001). The SUVmax ratio of [68Ga]FAPI was significantly different from [\(^{18}\)F] FDG (p = 0.03). Mean TBRmax for the [\(^{68}\)Ga]Ga-FAPI-4 tracer was markedly higher in comparison to [\(^{18}\)F]FDG (10.90 vs. 4.11).
Conclusion
Non-invasive imaging of FAP expression by [\(^{68}\)Ga]FAPI PET/CT resulted in a better visual detection of the malignant primary in CUP, as compared to [\(^{18}\)F]FDG imaging. However, the detection rate of lymph node metastases was inferior, presumably due to low FAP expression in small metastases. Nevertheless, by offering a detection method for primary tumors with the potential of lower false positive rates and thus avoiding biopsies, patients with CUP syndrome may benefit from [\(^{68}\)Ga]FAPI PET/CT imaging.
Human eye gaze conveys an enormous amount of socially relevant information, and the rapid assessment of gaze direction is of particular relevance in order to adapt behavior accordingly. Specifically, previous research demonstrated evidence for an advantage of processing direct (vs. averted) gaze. The present study examined discrimination performance for gaze direction (direct vs. averted) under controlled presentation conditions: Using a backward-masking gaze-discrimination task, photographs of faces with direct and averted gaze were briefly presented, followed by a mask stimulus. Additionally, effects of facial context on gaze discrimination were assessed by either presenting gaze direction in isolation (i.e., by only showing the eye region) or in the context of an upright or inverted face. Across three experiments, we consistently observed a facial context effect with highest discrimination performance for faces presented in upright position, lower performance for inverted faces, and lowest performance for eyes presented in isolation. Additionally, averted gaze was generally responded to faster and with higher accuracy than direct gaze, indicating an averted-gaze advantage. Overall, the results suggest that direct gaze is not generally associated with processing advantages, thereby highlighting the important role of presentation conditions and task demands in gaze perception.
Background
International consensus criteria (ICC) have redefined borderline resectability for pancreatic ductal adenocarcinoma (PDAC) according to three dimensions: anatomical (BR-A), biological (BR-B), and conditional (BR-C). The present definition acknowledges that resectability is not just about the anatomic relationship between the tumour and vessels but that biological and conditional dimensions also are important.
Methods
Patients’ tumours were retrospectively defined borderline resectable according to ICC. The study cohort was grouped into either BR-A or BR-B and compared with patients considered primarily resectable (R). Differences in postoperative complications, pathological reports, overall (OS), and disease-free survival were assessed.
Results
A total of 345 patients underwent resection for PDAC. By applying ICC in routine preoperative assessment, 30 patients were classified as stage BR-A and 62 patients as stage BR-B. In total, 253 patients were considered R. The cohort did not contain BR-C patients. No differences in postoperative complications were detected. Median OS was significantly shorter in BR-A (15 months) and BR-B (12 months) compared with R (20 months) patients (BR-A vs. R: p = 0.09 and BR-B vs. R: p < 0.001). CA19-9, as the determining factor of BR-B patients, turned out to be an independent prognostic risk factor for OS.
Conclusions
Preoperative staging defining surgical resectability in PDAC according to ICC is crucial for patient survival. Patients with PDAC BR-B should be considered for multimodal neoadjuvant therapy even if considered anatomically resectable.

Ciliary neurotrophic factor (Cntf) acts as a differentiation and survival factor for different types of neurons and glial cells. It is expressed by peripheral Schwann cells and astrocytes in the central nervous system and mediates its effects via a receptor complex involving CntfRα, LifRß and gp130, leading to downstream activation of Stat3. Recent studies by our group have shown that Cntf modulates neuronal microtubule dynamics via Stat3/stathmin interaction. In a mouse model for motor neuron disease, i.e. pmn, Cntf is able to rescue axonal degeneration through Stat3/stathmin signaling. While these findings suggest a role of Cntf in controlling axonal functions in the neuromuscular system, additional data indicate that Cntf might also play a role in synaptic plasticity in the hippocampus. Electrophysiological recordings in hippocampal organotypic cultures and acute slices revealed a deficit in long-term potentiation (LTP) in Cntf -/- mice. This deficit was rescued by 24 h stimulation with Cntf, combined with an acute application of Cntf during LTP-measurements indicating that Cntf is both necessary and sufficient for hippocampal LTP, and possibly synaptic plasticity. Therefore, Cntf knockout mice were investigated to elucidate this possible role of Cntf in hippocampal LTP and synaptic plasticity.
First, we validated the presence of Cntf in the target tissue: in the hippocampus, Cntf was localized in Gfap-positive astrocytes surrounding small blood vessels in the fissure and in meningeal areas close to the dentate gyrus. Laser micro-dissection and qPCR analysis showed a similar distribution of Cntf-coding mRNA validating the obtained immunofluorescent results. Despite the strong LTP deficit in organotypic cultures, in vivo behavior of Cntf -/- mice regarding hippocampus-dependent learning and anxiety-related paradigms was largely inconspicuous. However, western blot analysis of hippocampal organotypic cultures revealed a significant reduction of pStat3 levels in Cntf -/- cultures under baseline conditions, which in turn were elevated upon Cntf stimulation. In order to resolve and examine synaptic structures we turned to in vitro analysis of cultured hippocampal neurons which indicated that pStat3 is predominantly located in the presynapse. In line with these findings, presynapses of Cntf -/- cultures were reduced in size and when in contact to astrocytes, contained less pStat3 immunoreactivity compared to presynapses in wildtype cultures.
In conclusion, our findings hypothesize that despite of a largely inconspicuous behavioral phenotype of Cntf -/- mice, Cntf appears to have an influence on pStat3 levels at hippocampal synapses. In a next step these two key questions need to be addressed experimentally: 1) is there a compensatory mechanism by members of the Cntf family, possibly downstream of pStat3, which explains the in vivo behavioral results of Cntf -/- mice and can likewise account for the largely inconspicuous phenotype in CNTF-deficient humans? 2) How exactly does Cntf influence LTP through Stat3 signaling? To unravel the underlying mechanism further experiments should therefore investigate whether microtubule dynamics downstream of Stat3 and stathmin signaling are involved in the Cntf-induced modulation of hippocampal synaptic plasticity, similar to as it was shown in motoneurons.
Objectives
Trauma evaluation of extremities can be challenging in conventional radiography. A multi-use x-ray system with cone-beam CT (CBCT) option facilitates ancillary 3-D imaging without repositioning. We assessed the clinical value of CBCT scans by analyzing the influence of additional findings on therapy.
Methods
Ninety-two patients underwent radiography and subsequent CBCT imaging with the twin robotic scanner (76 wrist/hand/finger and 16 ankle/foot/toe trauma scans). Reports by on-call radiologists before and after CBCT were compared regarding fracture detection, joint affliction, comminuted injuries, and diagnostic confidence. An orthopedic surgeon recommended therapy based on reported findings. Surgical reports (N = 52) and clinical follow-up (N = 85) were used as reference standard.
Results
CBCT detected more fractures (83/64 of 85), joint involvements (69/53 of 71), and multi-fragment situations (68/50 of 70) than radiography (all p < 0.001). Six fractures suspected in radiographs were ruled out by CBCT. Treatment changes based on additional information from CBCT were recommended in 29 patients (31.5%). While agreement between advised therapy before CBCT and actual treatment was moderate (κ = 0.41 [95% confidence interval 0.35–0.47]; p < 0.001), agreement after CBCT was almost perfect (κ = 0.88 [0.83–0.93]; p < 0.001). Diagnostic confidence increased considerably for CBCT studies (p < 0.001). Median effective dose for CBCT was 4.3 μSv [3.3–5.3 μSv] compared to 0.2 μSv [0.1–0.2 μSv] for radiography.
Conclusions
CBCT provides advantages for the evaluation of acute small bone and joint trauma by detecting and excluding extremity fractures and fracture-related findings more reliably than radiographs. Additional findings induced therapy change in one third of patients, suggesting substantial clinical impact.
Purpose
We used exact matching for a highly balanced comparison of ab interno trabeculectomy (AIT) with the trabectome to trabeculectomy with mitomycin C (TRAB).
Methods
A total of 5485 patients who underwent AIT were exact-matched to 196 TRAB patients by baseline intraocular pressure (IOP), number of glaucoma medications, and glaucoma type. Nearest-neighbor–matching was applied to age. Success was defined as a final IOP of less than 21 mmHg, IOP reduction of at least 20% reduction from baseline, and no secondary surgical interventions. Outcomes were measured at 1, 3, 6, 12, 18, and 24 months.
Results
A total of 165 AIT could be matched to 165 TRAB. The mean baseline IOP was 22.3 ± 5.6 mmHg, and the baseline number of glaucoma medications was 2.7 ± 1.1 in both groups. At 24 months, IOP was reduced to 15.8 ± 5.2 mmHg in AIT and 12.4 ± 4.7 mmHg in TRAB. IOP was lower than baseline at all visits (p < 0.01) and lower in TRAB than AIT (p < 0.01). Glaucoma medications were reduced to 2.1 ± 1.3 in AIT and 0.2 ± 0.8 in TRAB. Compared to baseline, patients used fewer drops postoperatively (p < 0.01) and more infrequently in TRAB than in AIT (p > 0.01). Secondary surgical interventions had the highest impact on success and became necessary in 15 AIT and 59 TRAB patients. Thirty-two challenging events occurred in TRAB and none in AIT.
Conclusion
Both AIT and TRAB reduced IOP and medications. This reduction was more significant in TRAB but at the expense of four times as many secondary interventions.
The putative attachment protein G of pneumonia virus of mice (PVM), a member of the Pneumoviruses, is an important virulence factor with so far ambiguous function in a virus-cell as well as in virus-host context. The sequence of the corresponding G gene is characterized by significant heterogeneity between and even within strains, affecting the gene and possibly the protein structure. This accounts in particular for the PVM strain J3666 for which two differing G gene organizations have been described: a polymorphism in nucleotide 65 of the G gene results in the presence of an upstream open reading frame (uORF) that precedes the main ORF in frame (GJ366665A) or extension of the major G ORF for 18 codons (GJ366665U). Therefore, this study was designed to analyse the impact of the sequence variations in the respective G genes of PVM strains J3666 and the reference strain 15 on protein expression, replication and virulence.
First, the controversy regarding the consensus sequence of PVM J3666 was resolved. The analysis of 45 distinct cloned fragments showed that the strain separated into two distinct virus populations defined by the sequence and structure of the G gene. This division was further supported by nucleotide polymorphisms in the neighbouring M and SH genes. Sequential passage of this mixed strain in the cell line standardly used for propagation of virus stocks resulted in selection for the GJ366665A-containing population in one of two experiments pointing towards a moderate replicative advantage. The replacement of the G gene of the recombinant PVM 15 with GJ366665A or GJ366665U, respectively, using a reverse genetic approach indicated that the presence of uORF within the GJ366665A significantly reduced the expression of the main G ORF on translational level while the potential extension of the ORF in GJ366665U increased G protein expression. In comparison, the effect of the G gene-structure on virus replication was inconsistent and dependent on cell line and type. While the presence of uORF correlated with a replication advantage in the standardly used BHK-21 cells and primary murine embryonic fibroblasts, replication in the murine macrophage cell line RAW 264.7 did not. In comparison, the GJ366665U variant was not associated with any effect on replication in cultured cells at all. Nonetheless, in-vivo analysis of the recombinant viruses associated the GJ366665U gene variant, and hence an increased G expression, with higher virulence whereas the GJ366665A gene, and therefore an impaired G expression, conferred an attenuated phenotype to the virus.
To extend the study to other G gene organizations, a recombinant PVM expressing a G protein without the cytoplasmic domain and for comparison a G-deletion mutant, both known to be attenuated in vivo, were studied. Not noticed before, this structure of the G gene was associated with a 75% reduction in G protein expression and a significant attenuation of replication in macrophage-like cells. This attenuation was even more prominent for the virus lacking G. Taking into consideration the higher reduction in G protein levels compared to the GJ366665A variant indicates that a threshold amount of G is required for efficient replication in these cells.
In conclusion, the results gathered indicated that the expression levels of the G protein were modulated by the sequence of the 5’ untranslated region of the gene. At the same time the G protein levels modulated the virulence of PVM.
As a promising biofabrication technology, extrusion-based bioprinting has gained significant attention in the last decade and major advances have been made in the development of bioinks. However, suitable synthetic and stimuli-responsive bioinks are underrepresented in this context. In this work, we described a hybrid system of nanoclay Laponite XLG and thermoresponsive block copolymer poly(2-methyl-2-oxazoline)-b-poly(2-n-propyl-2-oxazine) (PMeOx-b-PnPrOzi) as a novel biomaterial ink and discussed its critical properties relevant for extrusion-based bioprinting, including viscoelastic properties and printability. The hybrid hydrogel retains the thermogelling properties but is strengthened by the added clay (over 5 kPa of storage modulus and 240 Pa of yield stress). Importantly, the shear-thinning character is further enhanced, which, in combination with very rapid viscosity recovery (~ 1 s) and structure recovery (~ 10 s), is highly beneficial for extrusion-based 3D printing. Accordingly, various 3D patterns could be printed with markedly enhanced resolution and shape fidelity compared to the biomaterial ink without added clay.
Purpose
Local treatment of small well-differentiated rectal neuroendocrine tumors (NETs) is recommended by current guidelines. However, although several endoscopic methods have been established, the highest R0 rate is achieved by transanal endoscopic microsurgery (TEM). Since a recently published study about endoscopic full thickness resection (eFTR) showed a R0 resection rate of 100%, the aim of this study was to evaluate both methods (eFTR vs. TEM).
Methods
We retrospectively analyzed all patients with rectal NET treated either by TEM (1999–2018) or eFTR (2016–2019) in two tertiary centers (University Hospital Wuerzburg and Ulm). We analyzed clinical, procedural, and histopathological outcomes in both groups.
Results
Twenty-eight patients with rectal NET received local treatment (TEM: 13; eFTR: 15). Most tumors were at stage T1a and grade G1 or G2 (in the TEM group two G3 NETs were staged T2 after neoadjuvant chemotherapy). In both groups, similar outcomes for en bloc resection rate, R0 resection rate, tumor size, or specimen size were found. No procedural adverse events were noted. Mean procedure time in the TEM group was 48.9 min and 19.2 min in the eFTR group.
Conclusion
eFTR is a convincing method for local treatment of small rectal NETs combining high safety and efficacy with short interventional time.
