Durch einen auffällig hohen Anteil an Patienten mit Autoimmungastritis, die gleichzeitig Antikörper gegen Helicobacter pylori aufweisen, wurde eine enge pathogenetische Korrellation der beiden Krankheitsbilder vermutet. So macht eine Entstehungstheorie der Autoimmungastritis eine Supermutation der B-Zellen nach Antigenkontakt mit dem H.pylori für eine autoimmune Entgleisung dieser verantwortlich. Die molekulargenetischen Untersuchungen der antikörperkodierenden Gene und deren Mutationen zeigen jedoch, daß das Mutationsniveau in der Autoimmungastritis niedriger ist als dies in der H. pylori Gastritis. Die Theorie der durch Supermutationen entstehenden autoimmungastritis aus der H.pylori Gastritis konnte in dieser Arbeit nicht bestätigt werden. Auch wurde durch des Aufstellen genealogischer B-Zellklon Stammbäume eine Kompartimentierung der inflammatorischen Zellen nach anatomischen Regionen, wie es das histopathologisches Bild der beschriebenen Gastritiden vermuten ließen nicht darstellen.
We herein report the case of a 73‐year‐old male patient who was diagnosed with leukemic non‐nodal MCL. This patient had received six cycles of bendamustine, which resulted in a transient remission, and a second‐line therapy with ibrutinib, which unfortunately failed to induce remission. We started a treatment with single‐agent obinutuzumab at a dose of 20 mg on day 1, 50 mg on day 2‐4, 330 mg on day 5, and 1000 mg on day 6. The laboratory analysis showed a rapid decrease of leukocyte count. Four weeks later, we repeated the treatment with obinutuzumab at a dose of 1000 mg q4w and started a therapy with venetoclax at a dose of 400 mg qd, which could be increased to 800 mg qd from the third cycle. This combination therapy was well tolerated. The patient achieved a complete remission (CR) after three cycles of obinutuzumab and venetoclax. To date, the patient has a progression‐free survival of 17 months under ongoing obinutuzumab maintenance q4w. This is the first report about obinutuzumab and venetoclax induced CR in rituximab‐intolerant patient with an ibrutinib‐resistant MCL. This case suggests that obinutuzumab‐ and venetoclax‐based combination therapy might be salvage therapy in patients with ibrutinib‐resistant MCL.
Das kolorektale Karzinom stellt die dritthäufigste Tumorerkrankung weltweit dar. Die Risikofaktoren sind vielseitig und werden in exogene und endogene Faktoren eingeteilt. Eine wichtige Präventionsmaßnahme von Kolonkarzinom ist die komplette endoskopische Koloskopie, die ab dem 55. Lebensjahr empfohlen wird. Der Goldstandard zur Behandlung von Kolonkarzinom ist nach wie vor die chirurgische Tumorresektion mit mikroskopisch nachgewiesener Tumorfreiheit. Eine chirurgische Sanierung der Fernmetastasen, welche am häufigsten in der Leber vorkommen, ist bei betroffenen Patienten anzustreben. Eine adjuvante Chemotherapie wird je nach UICC-Stadium des Tumors durchgeführt. Im Gegensatz zur Behandlung einiger maligner Tumorerkrankungen ist der Einsatz von Antikörpern noch kein fester Bestandteil der Therapie von Kolonkarzinomen.
In dieser Arbeit wurde Untersuchungsmaterial von 41 Patienten mit Kolonkarzinom, die am Universitätsklinikum Würzburg in den Jahren 1997 bis 2012 behandelt wurden, analysiert. Dabei wurden Paraffinschnitte vom Primärtumor, regionalen Lymphknotenmetastasen und Lebermetastasen der einzelnen Patienten mit 2 verschiedenen monoklonalen IgM-Antikörpern, PAT-SM6 und PAT-LM1, gefärbt und mikroskopisch untersucht. Der Antikörper PAT-SM6 wurde aus einem an einem Magenkarzinom erkrankten Patienten isoliert und bindet an eine Isotyp-Form des 'Glucose-Regulated' Protein (GRP)-78PAT-SM6. Als Zielstruktur des PAT-LM1 Antikörpers wurde eine tumorspezifische Form von NONO (Non-POU domain-containing octamer-binding protein) identifiziert (NONOPAT-LM1). Für beide Rezeptor-Isoformen wurde nachgewiesen, dass sie nur auf malignen epithelialen Zellen, nicht aber auf gesunden Zellen exprimiert werden. Anhand dieser Arbeit konnte gezeigt werden, dass PAT-SM6 die Tumorzellen der Lebermetastasen stärker anfärbte als Zellen des Primärtumors. Für die PAT-LM1 Antikörperfärbung wurde ein ähnliches Resultat erzielt. In Bezug auf das Lebensalter der Patienten wiesen die Tumorzellen von älteren Patienten (ab dem 65. Lebensjahr) eine stärkere Antikörperbindung durch PAT-SM6 und PAT-LM1 auf. Interessant war auch die Feststellung, dass die Tumorzellen der Lebermetastasen von verstorbenen Patienten durch PAT-LM1 stärker gefärbt waren als die von zum Untersuchungszeitpunkt noch lebenden Patienten. Die Bindungsunterschiede zwischen PAT-SM6 und PAT-LM1 könnten neue diagnostische und therapeutische Möglichkeiten bei Kolonkarzinomen bieten und somit zukünftig eine individuelle Tumortherapie ermöglichen.
The subclassification of diffuse large B-cell lymphoma (DLBCL) into germinal center B-cell-like (GCB) and activated B-cell-like (ABC) subtypes has become mandatory in the 2017 update of the WHO classification of lymphoid neoplasms and will continue to be used in the WHO 5\(^{th}\) edition. The RNA-based Lymph2Cx assay has been validated as a reliable surrogate of high-throughput gene expression profiling assays for distinguishing between GCB and ABC DLBCL and provides reliable results from formalin-fixed, paraffin-embedded (FFPE) material. This test has been previously used in clinical trials, but experience from real-world routine application is rare. We routinely applied the Lymph2Cx assay to day-to-day diagnostics on a series of 147 aggressive B-cell lymphoma cases and correlated our results with the immunohistochemical subclassification using the Hans algorithm and fluorescence in situ hybridization findings using break-apart probes for MYC, BCL2, and BCL6. The routine use of the Lymph2Cx assay had a high technical success rate (94.6%) with a low rate of failure due to poor material and/or RNA quality. The Lymph2Cx assay was discordant with the Hans algorithm in 18% (23 of 128 cases). Discordant cases were mainly classified as GCB by the Hans algorithm and as ABC by Lymph2Cx (n = 11, 8.6%). Only 5 cases (3.9%) were classified as non-GCB by the Hans algorithm and as GCB by Lymph2Cx. Additionally, 5.5% of cases (n = 7) were left unclassified by Lymph2Cx, whereas they were defined as GCB (n = 4) or non-GCB (n = 3) by the Hans algorithm. Our data support the routine applicability of the Lymph2Cx assay.
In this thesis we have investigated the effect of NFAT (Nuclear Factor of Activated T Cell) transcription factors on the expression of Rag-(Recombination Activating Genes) genes in murine thymus. The protein products of Rag genes, RAG1 and RAG2, are critical for the recombination and generation of the TCR (T Cell Receptor) repertoire during thymocyte development, and their expression can be suppressed by the activity of NFAT factors. In thymus, the expression of Rag1 and Rag2 genes is induced at the double-negative (DN, CD4-8-) 3 stage, down-regulated at the DN4 stage, re-induced at the double-positive (DP, CD4+8+) stage, and suppressed again at the single-positive (SP, CD4+8- or CD4-8+) stage. Although it is known that TCR signaling suppresses the expression of Rag1 and Rag2 at the SP stage, the signals that mediate the Rag gene down-reulation remain elusive. Here we report that both the calcineurin-NFAT-signaling and MAPKinase signaling pathways, which are activated by TCR signaling during positive selection, mediate the Rag gene down-regulation in DP thymocytes. The calcineurin-NFAT pathway suppresses both the Rag1 and the Rag2 gene expression. This pathway has a stronger suppressive effect on the Rag1 than the Rag2 gene. A synergistic activity between the two NFAT factors NFATc2 and NFATc3 is essential for calcineurin-NFAT signaling to efficiently suppress the Rag gene expression in DP thymocytes. It is likely that the calcineurin-NFAT signaling down-regulates Rag gene expression by suppressing both the Rag anti-silencer element (ASE) activity and the Rag promoter activity. Similarly, MEK-ERK signaling of MAPK signaling pathway mediates the Rag gene suppression in DP thymocytes although the mechanism through which MEK-ERK mediates the Rag gene down-regulation has to be elucidated. In DN thymocytes, it appears that neither the calcineurin-NFAT signaling nor MAPK signaling is involved in the Rag gene down-regulation. However, a role for these two signaling pathways in the Rag gene up-regulation in DN thymocytes is not excluded. In DN thymocytes, pre-TCR signaling stimulates the expression both Nfatc1 and Nfatc2 genes but has no effect on Nfatc3 gene expression. In DN thymocytes, pre-TCR signaling activates Nfatc1α expression but not Nfatc1ß expression, i.e. the two promoters controling Nfatc1 gene xpression are differently controled by pre-TCR signals. Nfatc1α gene expression in DN thymocytes is mainly regulated by the MAPK signaling pathway because activation of Nfatc1α is mediated by MEK-ERK signaling but opposed by JNK signaling. Calcineuirn-NFAT and p38 signaling pathways are not involved in Nfatc1α promoter regulation in DN thymocytes. In DP thymocytes, TCR signaling up-regulates Nfatc1 and Nfatc2 expression but down-regulates Nfatc3 expression. In DP thymocytes, TCR signaling activates Nfatc1α expression. The activation of Nfatc1α in DP thymocytes is mediated by NFATc1, but not or to a less degree by NFATc2 and NFATc3. MEK-ERK, JNK, and p38 signaling pathways are involved in Nfatc1α gene activation in DP thymocytes, probably by activating NFAT trans-activation activity. All these findings illustrate that in thymocytes the expression of NFAT transcription factors – which are essential for thymic development - is controled at multiple levels.
CD9 is the best-studied member of the tetraspanin family of transmembrane proteins. It is involved in various fundamental cellular processes and its altered expression is a characteristic of malignant cells of different origins. Despite numerous investigations confirming its fundamental role, the heterogeneity of CD9 or other tetraspanin proteins was considered only to be caused by posttranslational modification, rather than alternative splicing. Here we describe the first identification of CD9 transcript variants expressed by cell lines derived from fetal rat brain cells. Variant mRNA-B lacks a potential translation initiation codon in the alternative exon 1 and seems to be characteristic of the tumorigenic BT cell lines. In contrast, variant mRNA-C can be translated from a functional initiation codon located in its extended exon 2, and substantial amounts of this form detected in various tissues suggest a contribution to CD9 functions. From the alternative sequence of variant C, a different membrane topology ( 5 transmembrane domains) and a deviating spectrum of functions can be expected.
Background: Primary cutaneous follicular B-cell lymphoma (PCFBCL) represents an indolent subtype of Non-Hodgkin’s lymphomas, being clinically characterized by slowly growing tumors of the skin and common cutaneous relapses, while only exhibiting a low propensity for systemic dissemination or fatal outcome. Up to now, only few studies have investigated underlying molecular alterations of PCFBCL with respect to somatic mutations. Objectives: Our aim was to gain deeper insight into the pathogenesis of PCFBCL and to delineate discriminatory molecular features of this lymphoma subtype. Methods: We performed hybridization-based panel sequencing of 40 lymphoma-associated genes of 10 cases of well-characterized PCFBCL. In addition, we included two further ambiguous cases of atypical B-cell-rich lymphoid infiltrate/B-cell lymphoma of the skin for which definite subtype attribution had not been possible by routine investigations. Results: In 10 out of 12 analyzed cases, we identified genetic alterations within 15 of the selected 40 target genes. The most frequently detected alterations in PCFBCL affected the TNFRSF14, CREBBP, STAT6 and TP53 genes. Our analysis unrevealed novel mutations of the BCL2 gene in PCFBCL. All patients exhibited an indolent clinical course. Both the included arbitrary cases of atypical B-cell-rich cutaneous infiltrates showed somatic mutations within the FAS gene. As these mutations have previously been designated as subtype-specific recurrent alterations in primary cutaneous marginal zone lymphoma (PCMZL), we finally favored the diagnosis of PCMZL in these two cases based on these molecular findings. Conclusions: To conclude, our molecular data support that PCFBCL shows distinct somatic mutations which may aid to differentiate PCFBCL from pseudo-lymphoma as well as from other indolent and aggressive cutaneous B-cell lymphomas. While the detected genetic alterations of PCFBCL did not turn out to harbor any prognostic value in our cohort, our molecular data may add adjunctive discriminatory features for diagnostic purposes on a molecular level.
Introduction: Large-cell transformation (LCT) of mycosis fungoides (MF) has been associated with a higher risk of relapse and progression and, consequently, restricted prognosis. Its molecular pathogenesis has not been elucidated yet. Materials and Methods: In order to address molecular mechanisms of LCT, we performed hybrid capture panel-based sequencing of skin biopsies from 10 patients suffering from MF with LCT versus 17 patients without LCT including follow-up biopsies during clinical course, respectively (51 samples in total). The analyzed patients were attributed to three different groups based on the presence of LCT and clinical behavior. Results: While indolent MF cases without LCT did not show pathogenic driver mutations, a high rate of oncogenic alterations was detected in patients with LCT and aggressive clinical courses. Various genes of different oncogenic signaling pathways, including the MAPK and JAK-STAT signaling pathways, as well as epigenetic modifiers were affected. A high inter-individual and distinctive intra-individual mutation diversity was observed. Oncogenic RAS mutations were exclusively detected in patients with LCT. Conclusion: Our data demonstrate that LCT transition of MF is associated with increased frequency of somatic mutations in cancer-associated genes. In particular, the activation of RAS signaling — together with epigenetic dysregulation — may crucially contribute to the molecular pathogenesis of the LCT phenotype, thus conveying its adverse clinical behavior.
Das follikuläre Lymphom (FL) wird nach der aktuellen Klassifikation der WHO (World Health Organization Classification of Lymphoid Tumours) anhand der Zahl der Zentroblasten in drei Grade und der Grad 3 weiter in 3A und 3B eingeteilt. Bis heute ist die Rolle der FL3B aufgrund der morphologischen und genetischen Unterschiede zu den anderen FL umstritten, es wird eine eigene Entität und Pathogenese des FL3B diskutiert. Durch das Verbundprojekt „Molekulare Mechanismen in malignen Lymphomen“ (MMML) Daten zu FISH-, Genexpressionsanalysen und immunhistochemischen Färbungen bearbeitet werden.
Diesen Daten zufolge sind FL3B in ihrer Genexpression nicht von FL3A trennbar. Es konnte jedoch eine Abgrenzung der FL1/2 zu den FL3A/B durch die erhöhte Expression von 13 Genen in den FL3A/B gefunden werden, von denen Homolog, double strand break repair nuclease (MRE11A), Topoisomerase II alpha (TOP2A) und Thioredoxin (TXN) schon zuvor im Rahmen von FL und NHL diskutiert wurden.
Der humane monoklonale IgM-Antikörper PAM-1 induziert in seiner monomeren Form sowohl in vivo als auch in vitro Apoptose an Magenkarzinomzellen. Er bindet hierbei an den post-transskriptionell modifizierten Rezeptor CFR-1, der auf nahezu allen epithelialen Karzinomzellen und deren Vorläuferläsionen exprimiert wird. In dieser Arbeit werden die intrazellulären Mechanismen nach PAM-1/CFR-1-Bin- dung anhand von Protein(de)phosphorylierungen und deren selektiver Hemmung näher charakterisiert. Die hierbei detektierten Protein(de)phosphorylierungen sind somit möglicherweise essentielle Bestandteile der durch PAM-1 ausgelösten Apoptose.
The Proteome Profiles of the Cerebellum of Juvenile, Adult and Aged Rats-An Ontogenetic Study

In this study, we searched for proteins that change their expression in the cerebellum (Ce) of rats during ontogenesis. This study focuses on the question of whether specific proteins exist which are differentially expressed with regard to postnatal stages of development. A better characterization of the microenvironment and its development may result from these study findings. A differential two-dimensional polyacrylamide gel electrophoresis (2DE) and matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF-MS) analysis of the samples revealed that the number of proteins of the functional classes differed depending on the developmental stages. Especially members of the functional classes of biosynthesis, regulatory proteins, chaperones and structural proteins show the highest differential expression within the analyzed stages of development. Therefore, members of these functional protein groups seem to be involved in the development and differentiation of the Ce within the analyzed development stages. In this study, changes in the expression of proteins in the Ce at different postnatal developmental stages (postnatal days (P) 7, 90, and 637) could be observed. At the same time, an identification of proteins which are involved in cell migration and differentiation was possible. Especially proteins involved in processes of the biosynthesis and regulation, the dynamic organization of the cytoskeleton as well as chaperones showed a high amount of differentially expressed proteins between the analyzed dates.
The proteome profiles of the olfactory bulb of juvenile, adult and aged rats - an ontogenetic study

Background:
In this study, we searched for proteins that change their expression in the olfactory bulb (oB) of rats during ontogenesis. Up to now, protein expression differences in the developing animal are not fully understood. Our investigation focused on the question whether specific proteins exist which are only expressed during different development stages. This might lead to a better characterization of the microenvironment and to a better determination of factors and candidates that influence the differentiation of neuronal progenitor cells.
Results:
After analyzing the samples by two-dimensional polyacrylamide gel electrophoresis (2DE) and matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF-MS), it could be shown that the number of expressed proteins differs depending on the developmental stages. Especially members of the functional classes, like proteins of biosynthesis, regulatory proteins and structural proteins, show the highest differential expression in the stages of development analyzed.
Conclusion:
In this study, quantitative changes in the expression of proteins in the oB at different developmental stages (postnatal days (P) 7, 90 and 637) could be observed. Furthermore, the expression of many proteins was found at specific developmental stages. It was possible to identify these proteins which are involved in processes like support of cell migration and differentiation.
Current guidelines recommend consolidation with autologous stem cell transplantation (autoSCT) after induction chemotherapy for most patients with peripheral T-cell lymphoma (PTCL). This assumption is based on five prospective phase II studies, three of which included <50 patients with limited follow-up. Here we present the final analysis of the prospective German study. The treatment regimen consisted of four to six cycles of CHOP chemotherapy followed by mobilizing therapy and stem cell collection. Patients in complete remission (CR) or partial remission (PR) underwent myeloablative chemo(radio)therapy and autoSCT. From January 2001 to July 2010, 111 patients were enrolled in the study. The main subgroups were PTCL not specified (n=42) and angioimmunoblastic T-cell lymphoma (n=37). Seventy-five (68%) of the 111 patients received transplantation. The main reason for not receiving autoSCT was progressive disease. In an intent-to-treat analysis, the complete response rate after myeloablative therapy was 59%. The estimated 5-year overall survival, disease-free survival and progression-free survival rates were 44%, 54% and 39%, respectively. The results of this study confirm that upfront autoSCT can result in long-term remissions in patients with all major subtypes of PTCL and therefore should be part of first-line therapy whenever possible.
Disclosing the CXCR4 expression in lymphoproliferative diseases by targeted molecular imaging

Chemokine ligand-receptor interactions play a pivotal role in cell attraction and cellular trafficking, both in normal tissue homeostasis and in disease. In cancer, chemokine receptor-4 (CXCR4) expression is an adverse prognostic factor. Early clinical studies suggest that targeting CXCR4 with suitable high-affinity antagonists might be a novel means for therapy. In addition to the preclinical evaluation of [\(^{68}\)Ga]Pentixafor in mice bearing human lymphoma xenografts as an exemplary CXCR4-expressing tumor entity, we report on the first clinical applications of [\(^{68}\)Ga]Pentixafor-Positron Emission Tomography as a powerful method for CXCR4 imaging in cancer patients. [\(^{68}\)Ga]Pentixafor binds with high affinity and selectivity to human CXCR4 and exhibits a favorable dosimetry. [\(^{68}\)Ga]Pentixafor-PET provides images with excellent specificity and contrast. This non-invasive imaging technology for quantitative assessment of CXCR4 expression allows to further elucidate the role of CXCR4/CXCL12 ligand interaction in the pathogenesis and treatment of cancer, cardiovascular diseases and autoimmune and inflammatory disorders.
C-X-C motif chemokine receptor 4 (CXCR4) and somatostatin receptors (SSTR) are overexpressed in gastro-entero-pancreatic neuroendocrine tumors (GEP-NET). In this study, we aimed to elucidate the feasibility of non-invasive CXCR4 positron emission tomography/computed tomography (PET/CT) imaging in GEP-NET patients using [\(^{68}\)Ga]Pentixafor in comparison to \(^{68}\)Ga-DOTA-D-Phe-Tyr3-octreotide ([\(^{68}\)Ga]DOTATOC) and \(^{18}\)F-fluorodeoxyglucose ([\(^{18}\)F]FDG). Twelve patients with histologically proven GEP-NET (3xG1, 4xG2, 5xG3) underwent [\(^{68}\)Ga]DOTATOC, [\(^{18}\)F]FDG, and [\(^{68}\)Ga]Pentixafor PET/CT for staging and planning of the therapeutic management. Scans were analyzed on a patient as well as on a lesion basis and compared to immunohistochemical staining patterns of CXCR4 and somatostatin receptors SSTR2a and SSTR5. [\(^{68}\)Ga]Pentixafor visualized tumor lesions in 6/12 subjects, whereas [\(^{18}\)F]FDG revealed sites of disease in 10/12 and [\(^{68}\)Ga]DOTATOC in 11/12 patients, respectively. Regarding sensitivity, SSTR-directed PET was the superior imaging modality in all G1 and G2 NET. CXCR4-directed PET was negative in all G1 NET. In contrast, 50% of G2 and 80% of G3 patients exhibited [\(^{68}\)Ga]Pentixafor-positive tumor lesions. Whereas CXCR4 seems to play only a limited role in detecting well-differentiated NET, increasing receptor expression could be non-invasively observed with increasing tumor grade. Thus, [\(^{68}\)Ga]Pentixafor PET/CT might serve as non-invasive read-out for evaluating the possibility of CXCR4-directed endoradiotherapy in advanced dedifferentiated SSTR-negative tumors.
Exon-4 Mutations in KRAS Affect MEK/ERK and PI3K/AKT Signaling in Human Multiple Myeloma Cell Lines

Approximately 20% of multiple myeloma (MM) cases harbor a point mutation in KRAS. However, there is still no final consent on whether KRAS-mutations are associated with disease outcome. Specifically, no data exist on whether KRAS-mutations have an impact on survival of MM patients at diagnosis in the era of novel agents. Direct blockade of KRAS for therapeutic purposes is mostly impossible, but recently a mutation-specific covalent inhibitor targeting KRAS\(^{p.G12C}\) entered into clinical trials. However, other KRAS hotspot-mutations exist in MM patients, including the less common exon-4 mutations. For the current study, the coding regions of KRAS were deep-sequenced in 80 newly diagnosed MM patients, uniformely treated with three cycles of bortezomib plus dexamethasone and cyclophosphamide (VCD)-induction, followed by high-dose chemotherapy and autologous stem cell transplantation. Moreover, the functional impact of KRAS\(^{p.G12A}\) and the exon-4 mutations p.A146T and p.A146V on different survival pathways was investigated. Specifically, KRAS\(^{WT}\), KRAS\(^{p.G12A}\), KRAS\(^{p.A146T}\), and KRAS\(^{p.A146V}\) were overexpressed in HEK293 cells and the KRAS\(^{WT}\) MM cell lines JJN3 and OPM2 using lentiviral transduction and the Sleeping Beauty vector system. Even though KRAS-mutations were not correlated with survival, all KRAS-mutants were found capable of potentially activating MEK/ERK- and sustaining PI3K/AKT-signaling in MM cells.
The treatment of Parkinson's disease by transplantation of dopaminergic (DA) neurons from human embryonic mesencephalic tissue is a promising approach. However, the origin of these cells causes major problems: availability and standardization of the graft. Therefore, the generation of unlimited numbers of DA neurons from various types of stem or progenitor cells has been brought into focus. A source for DA neurons might be conditionally immortalized progenitor cells. The temperature-sensitive immortalized cell line CSM14.1 derived from the mesencephalon of an embryonic rat has been used successfully for transplantation experiments. This cell line was analyzed by unbiased stereology of cell type specific marker proteins and 2D-gel electrophoresis followed by mass spectrometry to characterize the differentially expressed proteome. Undifferentiated CSM14.1 cells only expressed the stem cell marker nestin, whereas differentiated cells expressed GFAP or NeuN and tyrosine hydroxylase. An increase of the latter cells during differentiation could be shown. By using proteomics an explanation on the protein level was found for the observed changes in cell morphology during differentiation, when CSM14.1 cells possessed the morphology of multipolar neurons. The results obtained in this study confirm the suitability of CSM14.1 cells as an in vitro model for the study of neuronal and dopaminergic differentiation in rats.
Somatic mutations in protein kinase A catalytic α subunit (PRKACA) were found to be causative for 30-40% of cortisol-producing adenomas (CPA) of the adrenal gland, rendering PKA signalling constitutively active. In its resting state, PKA is a stable and inactive heterotetramer, consisting of two catalytic and two regulatory subunits with the latter inhibiting PKA activity. The human genome encodes three different PKA catalytic subunits and four different regulatory subunits that are preferentially expressed in different organs. In normal adrenal glands all regulatory subunits are expressed, while CPA exhibit reduced protein levels of the regulatory subunit IIβ. In this study, we linked for the first time the loss of RIIβ protein levels to the PRKACA mutation status and found the down-regulation of RIIβ to arise post-transcriptionally. We further found the PKA subunit expression pattern of different tumours is also present in the zones of the normal adrenal cortex and demonstrate that the different PKA subunits have a differential expression pattern in each zone of the normal adrenal gland, indicating potential specific roles of these subunits in the regulation of different hormones secretion.
We aimed to elucidate the diagnostic potential of the C-X-C motif chemokine receptor 4 (CXCR4)-directed positron emission tomography (PET) tracer \(^{68}\)Ga-Pentixafor in patients with poorly differentiated neuroendocrine carcinomas (NEC), relative to the established reference standard \(^{18}\)F-FDG PET/computed tomography (CT). In our database, we retrospectively identified 11 treatment-naïve patients with histologically proven NEC, who underwent \(^{18}\)F-FDG and CXCR4-directed PET/CT for staging and therapy planning. The images were analyzed on a per-patient and per-lesion basis and compared to immunohistochemical staining (IHC) of CXCR4 from PET-guided biopsies. \(^{68}\)Ga-Pentixafor visualized tumor lesions in 10/11 subjects, while \(^{18}\)F-FDG revealed sites of disease in all 11 patients. Although weak to moderate CXCR4 expression could be corroborated by IHC in 10/11 cases, \(^{18}\)F-FDG PET/CT detected significantly more tumor lesions (102 vs. 42; total lesions, n = 107; p < 0.001). Semi-quantitative analysis revealed markedly higher 18F-FDG uptake as compared to \(^{68}\)Ga-Pentixafor (maximum and mean standardized uptake values (SUV) and tumor-to-background ratios (TBR) of cancerous lesions, SUVmax: 12.8 ± 9.8 vs. 5.2 ± 3.7; SUVmean: 7.4 ± 5.4 vs. 3.1 ± 3.2, p < 0.001; and, TBR 7.2 ± 7.9 vs. 3.4 ± 3.0, p < 0.001). Non-invasive imaging of CXCR4 expression in NEC is inferior to the reference standard \(^{18}\)F-FDG PET/CT.
TP53 mutations have been associated with anaplasia in Wilms tumour, which conveys a high risk for relapse and fatal outcome. Nevertheless, TP53 alterations have been reported in no more than 60% of anaplastic tumours, and recent data have suggested their presence in tumours that do not fulfil the criteria for anaplasia, questioning the clinical utility of TP53 analysis. Therefore, we characterized the TP53 status in 84 fatal cases of Wilms tumour, irrespective of histological subtype. We identified TP53 alterations in at least 90% of fatal cases of anaplastic Wilms tumour, and even more when diffuse anaplasia was present, indicating a very strong if not absolute coupling between anaplasia and deregulation of p53 function. Unfortunately, TP53 mutations do not provide additional predictive value in anaplastic tumours since the same mutation rate was found in a cohort of non-fatal anaplastic tumours. When classified according to tumour stage, patients with stage I diffuse anaplastic tumours still had a high chance of survival (87%), but this rate dropped to 26% for stages II–IV. Thus, volume of anaplasia or possible spread may turn out to be critical parameters. Importantly, among non-anaplastic fatal tumours, 26% had TP53 alterations, indicating that TP53 screening may identify additional cases at risk. Several of these non-anaplastic tumours fulfilled some criteria for anaplasia, for example nuclear unrest, suggesting that such partial phenotypes should be under special scrutiny to enhance detection of high-risk tumours via TP53 screening. A major drawback is that these alterations are secondary changes that occur only later in tumour development, leading to striking intratumour heterogeneity that requires multiple biopsies and analysis guided by histological criteria. In conclusion, we found a very close correlation between histological signs of anaplasia and TP53 alterations. The latter may precede development of anaplasia and thereby provide diagnostic value pointing towards aggressive disease.
The introduction of new therapeutic agents has revolutionized the treatment of metastatic melanoma. The approval of adjuvant anti‐programmed death‐1 monotherapy with nivolumab or pembrolizumab, and dabrafenib plus trametinib has recently set a new landmark in the treatment of stage III melanoma. Now, clinical trials have shown that immune checkpoint blockade can be performed in a neoadjuvant setting, an approach established as a standard therapeutic approach for other tumour entities such as breast cancer. Recent studies suggest that a pathological response achieved by neoadjuvant immunotherapy is associated with long‐term tumour control and that short neoadjuvant application of checkpoint inhibitors may be superior to adjuvant therapy. Most recently, neoadjuvant ipilimumab plus nivolumab in stage III melanoma was reported. With two courses of dose‐optimized ipilimumab (1 mg kg−1) combined with nivolumab (3 mg kg−1), pathological responses were observed in 77% of patients, while only 20% of patients experienced grade 3 or 4 adverse events. However, the neoadjuvant trials employing combined immune checkpoint blockade conducted so far have excluded patients with in transit metastases, a common finding in stage III melanoma. Here we report four patients with in transit metastases or an advanced primary tumour who have been treated with neoadjuvant ipilimumab plus nivolumab according to the OpACIN‐neo trial scheme (arm B). All patients achieved radiological disease control and a pathological response. None of the patients has relapsed so far.
Due to the rapidly increasing development and use of cellular products, there is a rising demand for non-animal-based test platforms to predict, study and treat undesired immunity. Here, we generated human organotypic skin models from human biopsies by isolating and expanding keratinocytes, fibroblasts and microvascular endothelial cells and seeding these components on a collagen matrix or a biological vascularized scaffold matrix in a bioreactor. We then were able to induce inflammation-mediated tissue damage by adding pre-stimulated, mismatched allogeneic lymphocytes and/or inflammatory cytokine-containing supernatants histomorphologically mimicking severe graft versus host disease (GvHD) of the skin. This could be prevented by the addition of immunosuppressants to the models. Consequently, these models harbor a promising potential to serve as a test platform for the prediction, prevention and treatment of GvHD. They also allow functional studies of immune effectors and suppressors including but not limited to allodepleted lymphocytes, gamma-delta T cells, regulatory T cells and mesenchymal stromal cells, which would otherwise be limited to animal models. Thus, the current test platform, developed with the limitation that no professional antigen presenting cells are in place, could greatly reduce animal testing for investigation of novel immune therapies.
Epstein-Barr virus (EBV) is best known for infection of B cells, in which it usually establishes an asymptomatic lifelong infection, but is also associated with the development of multiple B cell lymphomas. EBV also infects epithelial cells and is associated with all cases of undifferentiated nasopharyngeal carcinoma (NPC). EBV is etiologically linked with at least 8% of gastric cancer (EBVaGC) that comprises a genetically and epigenetically distinct subset of GC. Although we have a very good understanding of B cell entry and lymphomagenesis, the sequence of events leading to EBVaGC remains poorly understood. Recently, ephrin receptor A2 (EPHA2) was proposed as the epithelial cell receptor on human cancer cell lines. Although we confirm some of these results, we demonstrate that EBV does not infect healthy adult stem cell-derived gastric organoids. In matched pairs of normal and cancer-derived organoids from the same patient, EBV only reproducibly infected the cancer organoids. While there was no clear pattern of differential expression between normal and cancer organoids for EPHA2 at the RNA and protein level, the subcellular location of the protein differed markedly. Confocal microscopy showed EPHA2 localization at the cell-cell junctions in primary cells, but not in cancer cell lines. Furthermore, histologic analysis of patient tissue revealed the absence of EBV in healthy epithelium and presence of EBV in epithelial cells from inflamed tissue. These data suggest that the EPHA2 receptor is not accessible to EBV on healthy gastric epithelial cells with intact cell-cell contacts, but either this or another, yet to be identified receptor may become accessible following cellular changes induced by inflammation or transformation, rendering changes in the cellular architecture an essential prerequisite to EBV infection.
Genetische Veränderungen von Thymomen, insbesondere rekurrente Aberrationen, sind bislang unbekannt. In dieser Dissertation wurden 13 WHO Typ A Thymome, 23 WHO Typ B3 Thymome sowie 12 primäre Plattenepithelkarzinome (WHO Typ C Thymome) sowie einige seltene Primärtumoren des Thymus mittels Komparativer genomischer Hybridisierung untersucht. Mit Ausnahme eines einzigen Falles zeigten WHO Typ A Thymome keine chromosomalen Gewinne oder Verluste. Im Gegensatz dazu fanden sich bei allen Thymomen des Typs B3 genetische Aberrationen. Der Gewinn von 1q trat in 15 (65%), Gewinne von Chromosom 16 und Xq traten in jeweils 3 (13%) Fällen auf. Verluste fanden sich auf Chromosom 6 in 10 Fällen (43%) und an 13q in 6 Fällen (23%). Die häufigsten Gewinne bei primären Plattenepithelkarzinomen fanden sich auf 1q in 7 Fällen (58%), auf 17q in 4 Fällen (33%) und auf Chromosom 5 und 18 in jeweils 3 Fällen (25%). Rekurrente Verluste traten bei dieser Entität auf Chromosom 16q in 8 Fällen (67%), auf Chromosom 6 in 6 Fällen (50%), auf Chromosom 3 in 4 Fällen (33%) und auf den Chromosomen 13q sowie 17p in jeweils 3 Fällen (25%) auf. Die Dissertation zeigt, daß histologisch unterschiedliche Thymome mit unterschiedlichen, rekurrenten Aberrationen assoziiert sind. WHO Typ A Thymome weisen nur vereinzelt genetische Aberrationen auf. Thymome vom WHO Typ A und Typ B3 weisen verschiedene genetische Phänotypen auf, während Thymome vom Typ B3 sowie die primären Plattenepithelkarzinome des Thymus gemeinsame genetische Aberrationen zeigen. Neben den pathogenetischen Aspekten weist der beobachtete Verlust von Chromosom 6, auf dem sich die Genloci der HLA-Moleküle befinden, bei WHO Typ B3 Thymomen auch auf eine Rolle bei der Entstehung paraneoplastischer Autoimmunphänomene wie der Myasthenia Gravis hin.
Background:
Conventional parameters including Ki67, hormone receptor and Her2/neu status are used for risk stratification for breast cancer. The serine protease urokinase plasminogen activator (uPA) and the plasminogen activator inhibitor type-1 (PAI-1) play an important role in tumour invasion and metastasis. Increased concentrations in tumour tissue are associated with more aggressive potential of the disease. Multigene tests provide detailed insights into tumour biology by simultaneously testing several prognostically relevant genes. With OncotypeDX\(^{®}\), a panel of 21 genes is tested by means of quantitative real-time polymerase chain reaction.
The purpose of this pilot study was to analyse whether a combination of Ki67 and uPA/PAI-1 supplies indications of the result of the multigene test.
Methods:
The results of Ki67, uPA/PAI-1 and OncotypeDX\(^{®}\) were analysed in 25 breast carcinomas (luminal type, pT1/2, max pN1a, G2). A statistical and descriptive analysis was performed.
Results:
With a proliferation index Ki67 of < 14%, the recurrence score (RS) from the multigene test was on average in the low risk range, with an intermediate RS usually resulting if Ki67 was > 14%. Not elevated values of uPA and PAI-1 showed a lower rate of proliferation (average 8.5%) than carcinomas with an increase of uPA and/or PAI-1 (average 13.9%); p = 0.054, Student’s t-test. When Ki67 was > 14% and uPA and/or PAI-1 was raised, an intermediate RS resulted. These differences were significant when compared to cases with Ki67 < 14% with non-raised uPA/PAI-1 (p < 0.03, Student’s t-test). Without taking into account the proliferative activity, an intermediate RS was also verifiable if both uPA and PAI-1 showed raised values.
Conclusion:
A combination of the values Ki67 and uPA/PAI-1 tended to depict the RS to be expected. From this it can be deduced that an appropriate analysis of this parameter combination may be undertaken before the multigene test in routine clinical practice. The increasing cost pressure makes it necessary to base the implementation of a multigene test on ancillary variables and to potentially leave it out if not required in the event of a certain constellation of results (Ki67 raised, uPA and PAI-1 raised).
Die Unterscheidung zwischen körpereigenen und körperfremden Strukturen ist eine grundlegende Herausforderung der spezifischen Immunantwort. Pathologische Veränderungen dieser Abgrenzung können zu schwerwiegenden Autoimmunerkrankungen wie beispielsweise Diabetes Mellitus, Rheumatischer Arthritis oder Multipler Sklerose führen. Um unerwünschte (Auto-) Immunreaktionen zu verhindern, existieren verschiedene Formen von peripheren Toleranzmechanismen, die durch viele Transkriptionsfaktoren wie z. B. ICER (inducible cAMP early repressor), NFAT (nuclear factor of activated T cells) und Foxp3 (forkhead box protein p3) kontrolliert werden. Foxp3+ regulatorische T-Zellen (Tregs) sind spezialisierte immun-suppressive Lymphozyten, welche die Aktivierung anderer Immunzellen unterdrücken können. Einer der möglichen Mechanismen ist der Transfer zyklischen Adenosin-Monophosphats (cAMP) von Tregs in konventionelle T- und B-Lymphozyten. Die erhöhte intrazelluläre Konzentration an cAMP führt in Effektorzellen zur Induktion und Kerntranslokation von ICER. Der transkriptionelle Repressor ICER supprimiert die Expression vieler NFAT-regulierter Gene und hemmt darüber hinaus die Induktion der NFATc1/αA-Isoform selbst. Diese Isoform wird speziell in pro-inflammatorischen Effektorzellen hochreguliert und ist maßgeblich an deren spezifischem transkriptionellen Programm beteiligt. Foxp3 ist ein zentraler Faktor für die Bildung und Funktion sowohl Thymus-generierter nTregs als auch peripher (TGFβ-) induzierter iTregs. Die Kontrolle des Foxp3-Gens wird in iTregs – überraschenderweise aber nicht in nTregs – durch NFAT-Faktoren reguliert. Allerdings hemmt Foxp3 durch eine negative Rückkopplung wiederum die Induktion und Aktivität von NFATc1/αA. Dies stellt somit ein weiteres Regulativ dar, wobei Foxp3 nicht nur die Plastizität, sondern auch die Funktion von immun-suppressiven T-Zellen steuert. Zusätzlich regulieren die verschiedenen NFAT-Faktoren auch die Antigen präsentierenden dendritischen Zellen (DCs). Während NFATc1 und NFATc2 die Differenzierung und Proliferation von DCs beeinflussen, reguliert NFATc3 deren Zytokinexpression und steuert indirekt auch die nachfolgende T-Zell-Immunantwort. Die Kontrolle der Genregulation in Immunzellen durch die Transkriptionsfaktoren ICER, NFAT und Foxp3 erfüllt somit spezifische Funktionen der Immunität, reguliert aber gleichzeitig wichtige Aspekte der peripheren Toleranz, um schädliche (Auto-) Immunreaktionen zu verhindern.
Background
Investigation of the expression of an intestinal stem cell marker in esophageal adenocarcinomas (EAC) with and without Barrett's Esophagus (BE), with respect to a cancer stem cell (CSC) hypothesis.
Materials and methods
Expression of a putative intestinal stem cell marker LgR5 was analyzed in esophageal cancer specimen (n = 70: 41 EAC with BE, 19 EAC without BE, and n = 10 esophageal squamous-cell carcinomas, ESCC) and in the adenocarcinoma cell line OE-33. Ki-67 and Cdx-2 were co-labelled with LgR5 in double staining experiments. Immunhistochemical expression results were confirmed by RT-PCR and correlated with tumor stage and five-year survival rates.
Results
LgR5was found expressed in 35 of 41 (85%) EAC with BE and in 16 of 19 (81%) EAC without BE. By contrast, LgR5 was not found to be expressed in ESCC. Quantification of immunolabeling showed 15% LgR5+ cells in EAC with BE, 32% LgR5+ cells in adjacent BE and 13% in EAC without BE. Immunofluorescence double staining experiments with LgR5 and Ki-67 revealed a subpopulation (~5%) of proliferating LgR+/Ki-67+ cells. On mRNA-level, expression of LgR5 was higher in BE in comparison to EAC (p = 0.0159). High levels of LgR5 expression in BE associated EAC were associated with poorer survival in univariate analysis.
Conclusion
The stem cell marker LgR5 is expressed in EAC, irrespective of association with BE, and appears to have negative impact on survival. The subset of proliferating LgR5+ cells (<5%) might resemble rapidly cycling CSCs, which needs to be substantiated in further investigations.
Background
Renal cell carcinoma (RCC) is marked by high mortality rate. To date, no robust risk stratification by clinical or molecular prognosticators of cancer-specific survival (CSS) has been established for early stages. Transcriptional profiling of small non-coding RNA gene products (miRNAs) seems promising for prognostic stratification. The expression of miR-21 and miR-126 was analysed in a large cohort of RCC patients; a combined risk score (CRS)-model was constructed based on expression levels of both miRNAs.
Methods
Expression of miR-21 and miR-126 was evaluated by qRT-PCR in tumour and adjacent non-neoplastic tissue in n = 139 clear cell RCC patients. Relation of miR-21 and miR-126 expression with various clinical parameters was assessed. Parameters were analysed by uni- and multivariate COX regression. A factor derived from the z-score resulting from the COX model was determined for both miRs separately and a combined risk score (CRS) was calculated multiplying the relative expression of miR-21 and miR-126 by this factor. The best fitting COX model was selected by relative goodness-of-fit with the Akaike information criterion (AIC).
Results
RCC with and without miR-21 up- and miR-126 downregulation differed significantly in synchronous metastatic status and CSS. Upregulation of miR-21 and downregulation of miR-126 were independently prognostic. A combined risk score (CRS) based on the expression of both miRs showed high sensitivity and specificity in predicting CSS and prediction was independent from any other clinico-pathological parameter. Association of CRS with CSS was successfully validated in a testing cohort containing patients with high and low risk for progressive disease.
Conclusions
A combined expression level of miR-21 and miR-126 accurately predicted CSS in two independent RCC cohorts and seems feasible for clinical application in assessing prognosis.
Clear cell renal cell carcinoma (ccRCC) characterized by a tumor thrombus (TT) extending into the inferior vena cava (IVC) generally indicates poor prognosis. Nevertheless, the risk for tumor recurrence after nephrectomy and thrombectomy varies. An applicable and accurate prediction system to select ccRCC patients with TT of the IVC (ccRCC/TT) at high risk after nephrectomy is urgently needed, but has not been established up to now. To our knowledge, a possible role of microRNAs (miRs) for the development of ccRCC/TT or their impact as prognostic markers in ccRCC/TT has not been explored yet. Therefore, we analyzed the expression of the previously described onco-miRs miR-200c, miR-210, miR-126, miR-221, let-7b, miR-21, miR-143 and miR-141 in a study collective of 74 ccRCC patients. Using the expression profiles of these eight miRs we developed classification systems that accurately differentiate ccRCC from non-cancerous renal tissue and ccRCC/TT from tumors without TT. In the subgroup of 37 ccRCC/TT cases we found that miR-21, miR-126, and miR-221 predicted cancer related death (CRD) accurately and independently from other clinico-pathological features. Furthermore, a combined risk score based on the expression of miR-21, miR-126 and miR-221 was developed and showed high sensitivity and specificity to predict cancer specific survival (CSS) in ccRCC/TT. Using the combined risk score we were able to classify ccRCC/TT patients correctly into high and low risk cases. The risk stratification by the combined risk score (CRS) will benefit from further cohort validation and might have potential for clinical application as a molecular prediction system to identify high- risk ccRCC/TT patients.
Adamantiades-Behçet disease (ABD) is a chronic, multisystemic, recurrent, inflammatory vascular disorder of unknown etiology. Patients with symptoms initially appearing at the age of 16 or less are considered as cases of juvenile-onset ABD (JABD). JABD is relatively rare compared to ABD of adults, and only case reports and case studies have been published regarding this subtype of the disease. Epidemiology, clinical features, diagnosis and treatment of JABD are discussed in this review.
Contribution of adventitia-derived stem and progenitor cells to new vessel formation in tumors

Blocking tumor vascularization has not yet come to fruition to the extent it was hoped for, as angiogenesis inhibitors have shown only partial success in the clinic. We hypothesized that under- appreciated vascular wall-resident stem and progenitor cells (VW-SPCs) might be involved in tumor vascularization and influence effectiveness of anti-angiogenic therapy. Indeed, in patient samples, we observed that vascular adventitia-resident CD34\(^+\) VW-SPCs are recruited to tumors in situ from co-opted vessels. To elucidate this in detail, we established an ex vivo model using concomitant embedding of multi-cellular tumor spheroids (MCTS) and mouse aortic rings (ARs) into collagen
gels, similar to the so-called aortic ring assay (ARA). Moreover, ARA was modified by removing the ARs’ adventitia that harbors VW-SPCs. Thus, this model enabled distinguishing the contribution of VW-SPCs from that of mature endothelial cells (ECs) to new vessel formation. Our results show that the formation of capillary-like sprouts is considerably delayed, and their number and network formation were significantly reduced by removing the adventitia. Substituting iPSC-derived neural spheroids for MCTS resulted in distinct sprouting patterns that were also strongly influenced by the
presence or absence of VW-SPCs, also underlying the involvement of these cells in non-pathological vascularization. Our data suggest that more comprehensive approaches are needed in order to block all of the mechanisms contributing to tumor vascularization.
NFAT transcription factors play critical roles in gene transcription during immune responses. Besides regulation of lymphokine promoters in T lymphocytes, NFAT factors are also expressed in other cell types and regulate the activity of numerous genes that control the generation of cardiac septa and valves in embryonic heart, the formation of blood vessels, the outgrowth of neuronal axons and the differentiation of osteoclasts during bone formation [10, 24]. Here we show that the induction of NFATc/αA in effector T cells is controlled by a strong inducible promoter, P1. It results in splicing of exon 1 to exon 3 transcripts and, in concert with the activity of a poly A site downstream of exon 9, leads to the massive synthesis of NFATc/αA in effector Th1 cells. A second, weak promoter, P2, lies in front of exon 2 and directs the synthesis of longer NFAT β isoforms. Both P1 and P2 direct the synthesis of three different RNAs: αA, αB, αC and βA, βB, βC correspondingly. The B and C isoforms arise from alternative splicing and poly A addition at the distal site pA2. P1 but not P2 activity is autoregulated by NFAT factors which bind to two tandemly arranged NFAT sites within P1 and enhance its induction. In resting T cells, the NFATc1/β RNAs are the most prominent nfatc1 transcripts and their synthesis is reduced upon T-cell activation. However, following activation in primary effector T cells or in T-cell lines of human or murine origin, a 15–20-fold induction of NFATc1/αA RNA was detected, whereas only a 2–5-fold increase was observed for the NFATc1/αB or NFATc1/αC RNAs. Optimal induction of P1 promoter require involving of a persistent increase in free cytosolic Ca2+ induced by ionomycin, which stimulates the nuclear translocation and transcriptional activation of all NFATc factors and phorbol esters, which activate protein kinase C and other protein kinase pathways in T cells. This suggests that both TCR and co-receptor signals contribute to give full P1 nfatc1 induction. Because NFATc1/αA induction is unaffected in NFATc2+c3 double-deficient T cells, NFATc1 autoregulates its own synthesis by controlling P1 activity and NFATc1/αA induction. P1 promoter contains tandemly arranged NFAT core binding motif TGGAAA to witch bind monomeric NFATc1 proteins and numerous conservative binding sites of other transcriptional factors like CREB, Fos, ATF-2, Sp1, NF-kB and GATA suggesting complex multi-factor regulation of NFATc1 gene. We also highlight that initial phase of nfatc1 transcription in naive CD4+ T cells is controlled by the promoter P2 which is constitutively active in resting T cells. The activation of resting T cells results in a decrease of P2 and the induction of P1 activity and, under optimal conditions, in the predominant synthesis of NFATc1/αA in effector T cells. In addition to the high concentrations of poly A factors required for optimal pA1 function, the levels of transcription factors, in particular NFATs, must also increase for P1 induction. That could be explained by achievement of certain threshold levels for transcriptional activation. Finally, the altered transactivation potential of NFATc1/αA suggests a specific role for this NFATc1 protein in gene control, such as in Th1 effector cells where NFATc1/αA is synthesized at high concentrations.
In der vorliegenden Arbeit wurden die Expressionshäufigkeiten von MAGE-A-Antigenen, E -Cadherin, Laminin-5-gamma-2, MMP2 und MMP9 in Plattenepithelkarzinomen im Kopf- und Halsbereich untersuchen. Hierbei findet der Vergleich zwischen Primärtumoren und korrespondierenden Lymphknotenmetastasen besondere Beachtung. Um die Hypothese zu verifizieren, dass die o.g. Parameter einen signifikanten Einfluss auf die Progression und Metastasierung haben, wird der Zusammenhang zwischen den in der vorliegenden Arbeit gewonnenen Ergebnissen und diversen klinischen Parametern mittels Korrelationsanalyse untersucht.
Four molecules of the tumor suppressor p53 assemble to cooperatively bind proapoptotic target genes. The structural basis for cooperativity consists of interactions between adjacent DNA binding domains. Mutations at the interaction interface that compromise cooperativity were identified in cancer patients, suggesting a requirement of cooperativity for tumor suppression. We report on an analysis of cooperativity mutant p53(E177R) mice. Apoptotic functions of p53 triggered by DNA damage and oncogenes were abolished in these mice, whereas functions in cell-cycle control, senescence, metabolism, and antioxidant defense were retained and were sufficient to suppress development of spontaneous T cell lymphoma. Cooperativity mutant mice are nevertheless highly cancer prone and susceptible to different oncogene-induced tumors. Our data underscore the relevance of DNA binding cooperativity for p53-dependent apoptosis and tumor suppression and highlight cooperativity mutations as a class of p53 mutations that result in a selective loss of apoptotic functions due to an altered quaternary structure of the p53 tetramer.
Im Rahmen der vorliegenden Arbeit wurde eine detaillierte Analyse des Chromosom 17p-Status in DLBCL durchgeführt, welches das TSG TP53 beinhaltet. Eine monoallelische Deletion dieses Gens fand sich in 16% der Patienten (28/172), darüber hinaus lag in 3% (6/172) der Patienten ein Verlust in Chromosom 17p ohne Alteration des TP53-Lokus vor. Zur Untersuchung des zweiten TP53-Allels wurden an allen 28 Patienten mit sowie an 27 Patienten ohne TP53-Deletion Mutationsanalysen durchgeführt. Eine vollständige Inaktivierung des TP53-Gens durch Deletion und Mutation konnte in 46% der 17p-deletierten Fälle (13/28) gezeigt werden. In 54% der 17p-deletierten DLBCL (15/28) lag lediglich eine monoallelische TP53-Deletion ohne gleichzeitige Mutation des zweiten Allels vor. 26% der DLBCL mit nicht-deletiertem TP53 (7/27) zeigten eine monoallelische Mutation des TP53-Gens. Diese Ergebnisse deuteten auf weitere TSG in der chromosomalen Region 17p13 hin. Mit der Durchführung einer detaillierten Deletionsanalyse von Chromosom 17p13.1 bis 17p13.3 wurden folgende Ergebnisse erzielt. In insgesamt 41% der DLBCL mit nicht-deletiertem TP53-Gen (11/27) konnte eine minimal deletierte Region identifiziert werden, welche die chromosomale Bande 17p13.3 einschließlich des genetischen Lokus des TSG HIC1 betraf. Eine Deletion von TP53 war in allen untersuchten Fällen mit einem subtotalen Verlust des kurzen Armes von 17p einschließlich der o.g. minimal deletierten Region verbunden. Mehrere Hinweise deuten darauf hin, dass die Inaktivierung von HIC1 eine entscheidende Rolle in der Pathogenese von DLBCL einnimmt. In 55% der untersuchten DLBCL (30/55) wurde eine Hypermethylierung des HIC1-Promoters nachgewiesen. In 90% der HIC1-hypermethylierten DLBCL (27/30) lag zudem eine gleichzeitige Deletion des HIC1-Lokus vor, was eine biallelische Inaktivierung des TSG, analog der „two-hit“-Hypothese nach Knudson, nahe legte. Eine vollständige durch Hypermethylierung und Deletion verursachte HIC1-Inaktivierung konnte auch in sieben Fällen mit Wildtyp TP53-Status gezeigt werden. Die Überlebenszeit von Patienten mit einer gleichzeitigen HIC1- und TP53-Inaktivierung war im Vergleich zu Patienten mit alleiniger Inaktivierung von TP53 deutlich verkürzt. Es konnte somit gezeigt werden, dass in den untersuchten DLBCL, über die klinisch bedeutsame und prognostisch relevante Inaktivierung von TP53 hinaus, ein weiteres TSG unabhängig von oder in Kombination mit TP53 alteriert ist, und einen Einfluss auf die Überlebenszeit der Patienten hat.
Follikuläre Lymphome (FL) zählen zu den Non-Hodgkin-Lymphomen und stellen die
größte Untergruppe der B-Zell-Lymphome dar. Bedingt durch ihren meist indolenten
Verlauf werden sie oft erst in einem fortgeschrittenen klinischen Stadium III/IV
diagnostiziert und stellen dann eine systemische Erkrankung dar.
Gelegentlich wird in der histopathologischen Untersuchung eines befallenen
Lymphknotens eine nur partielle Infiltration beobachtet, die häufig auch in den
angeschlossenen Stagingmaßnahmen mit einer nur lokalen Tumorausbreitung (klinisches
Stadium I/II) assoziiert ist. Ein solches lokal begrenztes Stadium kann gemäß der
Standard-Behandlungsprotokolle mit einer alleinigen Strahlentherapie ausreichend
kontrolliert werden.
Ziel der vorliegenden Arbeit war es zum einen, eine mögliche Assoziation einer nur
partiellen Lymphknoteninfiltration beim FL mit einem lokal begrenzten klinischen Stadium
zu untersuchen. Zum anderen sollte die Inzidenz einer nur partiellen Lymphknoten-
Infiltration beim FL bestimmt werden.
Der Vergleich der Studienkohorte mit einer nur partiellen Lymphknoteninfiltration, definiert
als zumindest ein vollständig erhaltener Lymphfollikel, mit der Kontrollkohorte zeigte
einen hochsignifikanten Unterschied: In der Studienkohorte befanden sich 38 von 40 Fälle
(95%) in einem lokalen Stadium, wohingegen die Kontrollkohorte mit vollständiger
Lymphknoteninfiltration nur bei 10 von 49 Patienten (20%) ein lokales Krankheitsstadium
(p<0.001) aufwies.
Um die erhaltenen Ergebnisse zu validieren, wurden alle FL Grad 1-3A aus dem
exemplarischen Jahr 2001 untersucht. Hier zeigte sich in 34 Fällen (11 %) eine nur
partielle Infiltration. In allen 18 Fällen mit mindestens einem vollständig erhaltenen
reaktiven Keimzentrum lag in Übereinstimmung mit der initialen Studienkohorte ein lokales
Krankheitsstadium I/II vor (p<0.001).
Die erhaltenen Ergebnisse zeigen eindrücklich, dass follikuläre Lymphome mit einem nur
partiellen Befall der Lymphknoten häufig mit einem (noch) lokalen klinischen Stadium
assoziiert sind. In diesen Fällen käme eine alleinige Bestrahlung als Therapieoption in
Betracht.
In der vorliegenden Arbeit wurden einerseits zelltypspezifische Untersuchungen der mitochondrialen DNA zur Bestimmung der Deletionslast, als Marker für oxidativen Stress, andererseits neuroinflammations-assoziierte Genexpressions-Analysen am humanen post mortem Hirngewebe von Patienten mit unterschiedlichen Stadien der Alzheimer Erkrankung durchgeführt. Als Grundlage hierzu diente das noch nicht gänzlich aufgeschlüsselte Konzept der selektiven Vulnerabilität unterschiedlicher Hirnregionen. Dabei zeigte sich, dass der Hippocampus, eine auf lichtmikroskopischer Ebene sehr früh befallene Region, auch molekularbiologisch deutliche Unterschiede gegenüber resistenten Regionen wie z.B. dem Kleinhirn aufweist.
Die Rolle der DNA-Methylierung in der Kontrolle des MHC-II-spezifischen Typ-IV-Promotors in Thymomen
(2004)
Die Moleküle des „major histocompatibility complex“ (MHC) spielen in der thymischen T-Zell-Reifung eine zentrale Rolle. Durch Interaktion des T-Zell-Rezeptors auf der unreifen T-Zelle und den Co-Rezeptoren CD4 und CD8 mit dem MHC auf dem Thymusepithel werden die zwei wichtigsten Reifungs- und Selektionsprozesse, positive und negative Selektion, vermittelt. Störungen dieser Selektionsmechanismen können zu Immundefekten oder Autoimmunphänomenen führen. Die Regulierung der MHC IIExpression erfolgt hauptsächlich transkriptionell und wird vor allem durch den sogenannten Class II Transaktivator (CIITA) bestimmt. Die Transkription von CIITA wird über vier alternative, gewebsspezifische Promotoren durch alternatives splicing reguliert. Unter diesen Splicevarianten ist der CIITA Typ IV Promotor für die konstitutive MHC II-Expression in Thymusepithelzellen und für die IFN-g induzierte Expression in anderen Geweben verantwortlich. Thymome sind menschliche Tumoren des Thymusepithels, welche häufig mit einer paraneoplastischen Myasthenia gravis (MG) assoziiert sind. Die paraneoplastische MG zeigt eine starke positive Korrelation mit der Fähigkeit eines Thymoms, reife CD4+ T-Zellen zu produzieren und in das periphere Blut zu exportieren. Thymome weisen aus bislang ungeklärten Gründen häufig eine starke Reduktion der epithelialen MHC II- Expression auf. Ziel dieser Arbeit war die Beantwortung der Frage, ob eine DNA-Methylierung der Promotorregion des MHC-Klasse II Transaktivators (CIITA) Typ IV daran beteiligt ist. Hierzu wurden 27 Thymome mit sowohl hoher und als auch niedriger MHC II-Expression und 9 Normalthymi aus unterschiedlichen Altersgruppen untersucht. Die Ergebnisse zeigten, dass der CIITA Typ IV Promotor in kindlichen Kontrollthymi vollständig unmethyliert ist, während die Thymi erwachsener Kontrollpersonen eine vermehrte Methylierung aufweisen. Im Gegensatz dazu war der CIITA Typ IV in nahezu allen Thymomen stark hypermethyliert. Da aber kein signifikanter Unterschied in der Methylierung zwischen Thymomen mit hoher und solchen mit niedriger MHC II-Expression festgestellt werden konnte, lassen diese Ergebnisse vermuten, dass eine Hypermethylierung des CIITA Typ IV Promotors sehr wahrscheinlich nicht der alleinige Grund für die verminderte MHC II-Expression in Thymomen ist, sondern das auch andere Regulationsmechanismen wie zum Beispiel Histon-Acetylierung beteiligt sein könnten beteiligt sein könnten.
Die Entdeckung der Autoimmunkrankheit APECED führte zur Entdeckung des verantwortlichen Gens AIRE (autoimmune regulator), das einen Einblick in die Verhinderung oder Entstehung von Autoreaktivität im Thymus bietet. Durch Aktivierung des AIRE-Gens und Expression des AIRE-Proteins werden organspezifische Selbstantigene in medullären Thymusepithelzellen exprimiert und entweder direkt oder nach Transfer auf dendritsche Zellen unreifen T-Zellen präsentiert. Durch Elimination autoreaktiver Zellen entsteht Toleranz gegenüber den körpereigenen Geweben. Da das Immunsystem bei steigendem Alter tiefgreifende Veränderungen erfährt, die zum Teil durch die Thymusinvolution bedingt sind, ist es von Interesse, den Verlauf von AIRE über verschiedene Altersstufen hinweg zu untersuchen. Zielsetzungen dieser Arbeit waren daher die quantitative und räumliche Untersuchung der AIRE+ -Zellen im Alter und im Kontext einer prototypischen organspezifischen Autoimmunerkrankung, der Myasthenia gravis. Diese Fragestellungen wurden bei Normalthymi, entzündlich veränderten Thymi mit assoziierter Myasthenie und an B1-Thymomen untersucht. Als Ergebnis konnte erarbeitet werden, dass der Gehalt an AIRE+ -Zellen mit dem Alter abnimmt, wobei bis in die neunte Lebensdekade noch AIRE+ -Zellen vorhanden waren. Der Gehalt an AIRE+ Zellen stellte sich als stärker mit dem Lebensalter korreliert dar als beispielsweise die Thymusfläche. AIRE scheint damit ein mit der Thymusinvolution eng assoziiertes Gen zu sein. Es fand sich eine hochsignifikante Co-Lokalisation von AIRE+ Zellen und Hassall-Körperchen, die besonders durch die Untersuchungen von Typ B1 Thymomen mit und ohne Hassall-Körperchen bestätigt werden konnte. In Übereinstimmung mit aktuellen Forschungsergebnissen in Mausmodellen fand sich kein Zusammenhang zwischen der Anzahl oder der räumlichen Anordnung regulatorischer T-Zellen und AIRE+ -Zellen. Myoidzellen nahmen mit zunehmendem Alter ebenfalls ab. Ihre Rolle in der Entstehung der Myasthenie ist bei Patienten mit early und late onset wahrscheinlich unterschiedlich; die abnehmende Zahl an Myoidzellen stellt möglicherweise vor allem bei Patienten mit late onset einen Risikofaktor dar.
„Black esophagus“ oder „akute Ösophagusnekrose“ (AÖN) ist eine seltene Erkrankung, die sich makroskopisch durch eine zirkumferente Schwarzverfärbung der Ösophagusmukosa mit abruptem Ende am gastroösophagealen Übergang auszeichnet. Die genaue Pathogenese ist unbekannt; es werden multifaktorielle Einflüsse wie z. B. Säurereflux, Ischämie und verringerte Schutzmechanismen der Mukosa als mögliche Ursachen diskutiert.
Vorgestellt werden 2 Obduktionsfälle, die typische Befunde einer AÖN aufwiesen. Zusätzlich hatten Fall 1 eine Candida-Infektion und Fall 2 eine Appendizitis, sodass eine infektiöse Genese in beiden Fällen eine Rolle gespielt haben könnte.
In der vorliegenden Arbeit wurden der klinische Verlauf der Tumorerkrankung von 40 Kindern, die an einem Ependymom erkrankten, erfasst und 58 Tumorproben mittels immunhistochemischer Methoden untersucht. Im Mittelpunkt dieser Untersuchungen standen die Überprüfung der p53-Akkumulation, der Proliferationsaktivität mit Hilfe des Antikörpers MIB-1 sowie der Expression von HLA-DR durch Tumorzellen und eine Einschätzung der prognostischen Aussagekraft dieser Faktoren. Weiterhin wurde die EMA-Expression untersucht und eine Schwerpunkt auf die Analyse von Verteilung und Proliferationsaktivität der Zellen des Mikroglia(MG)/Makrophagen-Systems in kindlichen Ependymomen gelegt. Das mittlere Alter bei Diagnosestellung lag bei 61 Monaten (8 Monate bis 15 4/12 Jahre, Median: 41 Monate), eine Geschlechtspräferenz lag nicht vor. 22,5% der Primärtumoren waren supratentoriell, 60% infratentoriell und 17,5% spinal lokalisiert. 52,5% der Primärtumoren waren anaplastische Ependymome (WHO Grad III), 40% Ependymome (WHO Grad II), 7,5% myxopapilläre Ependymome (WHO Grad I). Eine totale Resektion konnte in 30,8% bei Primärtumoren und in 27,3% bei Rezidivtumoren erzielt werden. Das mittlere Nachbeochtungsintervall betrug 52 Monate (6-163 Monate), das mittlere progrssionsfreie Interval (PFS) 28 Monate und der mittlere Überlebenszeitraum (OS) 64 Monate. Tumorrezidive entwickelten sich mit einer Ausnahme eines Spätrezidives nach 84 Monaten innerhalb von 36 Monaten nach der operativen Tumorentfernung. Von den klinischen Variablen weist einzig das Resektionsausmaß eine prognostische Bedeutung auf (total vs. inkomplett: p=0.035 bezüglich des mittleren PFS, p=0.12 bezüglich des mittleren OS). Alle anderen klinischen Variablen, einschließlich des Tumorgrades nach WHO, zeigen keinen Einfluss auf den Verlauf der Tumorerkrankung. p53-Akkumulation kommt in 56,9% der untersuchten Tumorproben vor und korreliert mit dem Tumorgrad nach WHO, dem MIB-1 Labeling Index (LI) und der Lokalisation. Der MIB-1 LI zeigt eine große Streubreite (1,4% - 63,3%) und weist entsprechend des untersuchten Patientenkollektivs im Gegensatz zu Ependymomen des Erwachsenenalters einen hohen Mittelwert und Median auf (21,8% bzw. 19,8%). Der MIB-1 LI korreliert statistisch signifikant mit dem WHO-Graduierungssystem, der Lokalisation und wie bereits erwähnt mit der p53-Akkumulation. Unter den immunhistochemisch untersuchten Variablen kommt lediglich dem MIB-1 LI im untersuchten Patientenkollektiv eine gewisse prognostische Aussagekraft zu. Statistische Signifikanz wird allerdings nicht erreicht (MIB-1 LI <10% vs. MIB-1 LI >10%: p=0.17 bezüglich des mittleres PFS). Erstmals wurde in 80,4% der untersuchten ependymalen Tumoren auch eine Proliferation von Zellen des MG/Makrophagen-Systems nachgewiesen. Bei weitgehend konstant niedriger absoluter Anzahl proliferierender Zellen des MG/Makrophagen-Systems existiert kein Unterschied diesbezüglich zwischen den Tumorgraden wie kürzlich bei astrozytären Tumoren nachgewiesen. Die Verteilung der Zellen des MG/Makrophagen-Systems zeigte bei sehr heterogenem Verteilungsmuster häufig eine septale Anordnung insbesondere der amöboiden MG und der Makrophagen. Es existieren unterschiedliche Verteilungsmuster mikroglialer Zellen und Makrophagen, die sich aber anderen Parametern nicht zuordnen lassen mit der Ausnahme der MG/Makrophagen-Zelldichte in Primärtumoren. Obwohl eine HLA-DR Expression durch Tumorzellen auch in anderen Neoplasien des ZNS häufig vorkommt, wurden Ependymome diesbezüglich bisher nicht untersucht. In 52,6% aller Tumorproben finden sich in der vorliegenden Arbeit HLA-DR exprimierende Tumorzellen. Eine EMA-Expression zeigt sich in 84,5% der untersuchten Fälle und ist damit ein Charakteristikum ependymaler Neoplasien, das auch als differentialdiagnostisches Kriterium angesehen werden kann. Die Ergebnisse der vorliegenden Arbeit bekräftigen den hohen prognostischen Stellenwert einer totalen Resektion in der Therapie ependymaler Tumoren des Kindesalters. Weiterhin wird die Bedeutung der Proliferationsaktivität für den klinischen Verlauf der Tumorerkrankung unterstrichen. Von großem wissenschaftlichem Interesse ist weiterhin die Klärung der Ursache und Bedeutung der häufig anzutreffenden p53-Akkumulation sowie der immunologischen Bedeutung der Zellen des MG/Makrophagen-Systems im Rahmen der Tumorabwehr.
Purpose
Patients suffering from aggressive systemic peripheral lymphoma with primary central nervous system involvement (PCL) are a rare and sparsely investigated population. Recommended treatment regimens include a combination of intrathecal and systemic chemotherapy as well as whole brain radiotherapy while offering relatively poor survival.
Methods
We conducted a single-center retrospective study that analyzed safety and outcome of 4 + 4 cycles Rituximab (R)-CHOP and R-high-dose Methotrexate (HD-MTX) for newly diagnosed, transplant-eligible patients ("Ping-Pong"), followed by Cytarabine (AraC)/Thiotepa (TT), BCNU/TT, and autologous hematologic stem cell transplantation (aHSCT). We retrospectively analyzed a set of 16 patients with high-intermediate or high-risk IPI status.
Results
Overall response rate to Ping-Pong was 100% measured by CT/MRI, including 93.75% complete remissions after BCNU/TT followed by PBSCT. One patient failed to qualify for high-dose chemotherapy due to progression when receiving Cytarabine/TT. All patients experienced grade III adverse events, 3 of them a grade IV adverse event. Estimated progression-free survival is 93.75% after a 4.8-year follow-up currently.
Conclusion
Our study suggests high effectivity of R-CHOP with mid-cycle MTX with aHSCT consolidation towards acceptable OS results in this challenging patient population.
Marginalzonen-B-Zell-Lymphome vom MALT-Typ stellen die häufigste Form primär extranodaler Non-Hodgkin-Lymphome dar. Sie entstehen auf dem Boden einer chronischen infektiösen oder autoimmunen Entzündung in hierdurch sekundär erworbenem lymphatischen Gewebe. Eine charakteristische und die häufigste bei diesen Tumoren beobachtete Translokation ist die t(11;18)(q21;q21). Hierbei fusioniert API 2, ein Apoptoseinhibitor-Gen auf 11q21 mit MALT 1 auf 18q21, einer humanen Paracaspase. Mit dem Ziel, letztlich die Funktion und Rolle des durch die Translokation trunkierten MALT 1-Gens untersuchen zu können, wurde bei dieser Arbeit ein Vektorsystem konstruiert, mit dem das MALT 1-Bruchstück stabil und nachweislich in humane B-Zelllinien transfiziert werden kann. An und mit diesen Transfektanten können nun vielfältige Analysen durchgeführt werden, um weitere Einsichten in die Lymphompathogenese zu gewinnen. Im Rahmen dieser Arbeit wurden noch Untersuchungen zum Wachstumsverhalten und Zellzyklus von das trunkierte MALT 1-Gen überexprimierenden Zellen durchgeführt. Es zeigte sich, dass diese Zellen im Vergleich zu das intakte Gen exprimierenden Zellen stärker und konstanter proliferierten. Dies legte den Schluss nahe, dass in der Genstruktur vor dem Bruchpunkt proliferationshemmende und regulatorische Domänen liegen könnten, welche schließlich durch die Translokation eliminiert werden. Hierdurch könnte der verbleibende Anteil des Gens unreguliert zur Tumorgenese beitragen.
Zur Identifizierung geeigneter Routinemarker für die Prognose von Ependymompatienten führten wir immunhistochemische Untersuchungen und statistische Auswertungen an Ependymomen und Daten von 32 Erwachsenen und 23 pädiatrischen Patienten durch. Davon wurden bei drei Tumoren auch Rezidive untersucht, so dass insgesamt 59 Ependymome in die Untersuchung eingeschlossen wurden. Im Einzelnen handelte es sich um 11 myxopapilläre Ependymome, 6 Subependymome, 19 Ependymome und 23 anaplastische Ependymome. Die größten Fallgruppen bildeten pädiatrische Patienten unter drei Jahren und Erwachsene zwischen 50 und 70 Jahren. Bei Kindern war mit 45,8% die infratentorielle, bei Erwachsenen mit 65% die spinale Tumorlokalisation am häufigsten. Die untersuchten spinalen Ependymome entsprachen zu gleichen Teilen myxopapillären Ependymomen WHO Grad I und Ependymomen WHO Grad II. In supratentorieller Lage fanden sich mit 67% überwiegend anaplastische Ependymome WHO Grad III. Auch bei den infratentoriell gelegenen Ependymomen waren mit 63% die Mehrzahl anaplastische Ependymome, daneben fanden sich 29,6% Ependymome WHO Grad II. Beim Vergleich des von uns definierten und bestimmten Ki67-Scores als Zeichen für die Ependymomproliferation und der immunhistochemischen Positivität für HCK fiel nach Anwendung des Chi-Quadrat-Tests mit p=0,067 ein deutlicher Trend zu schwächerer punktförmiger Positivität bei höherem Ki67-Score auf. Dieser Trend setzte sich in der Erwachsenengruppe separat fort, während er in der Kindergruppe allein nicht nachweisbar war. In der Erwachsenengruppe war mit 28% ein deutlicher Anteil myxopapillärer Ependymome vorhanden, welche bei den Kindern nur 8% ausmachten.Möglicherweise spielt die veränderte HCK-Expression in der Subgruppe der myxopapillären Ependymome eine Rolle. Unsere Untersuchungen zeigten außerdem mit p=0,057 einen deutlichen Trend zu längerem Überleben bei immunohistochemischer DBC1-Negativität. Die Multivarianzanalyse mittels Cox-Regression wies eine Positivität für DBC1 als unabhängigen Risikofaktor für eine kürzere Überlebenszeit nach. Des Weiteren konnte eine mit p=0,013 signifikante Korrelation zwischen immunhistochemischer Positivität für DBC1 und höherem Ki67-Score gezeigt werden. Auch mit höherem WHO-Grad korrelierte die DBC1-Positivität mit p=0,009. Besonders infratentoriell gelegene Ependymome zeigten DBC1-Reaktivität. Hier treten bekannterweise häufiger anaplastische Ependymome mit höherem Proliferationsindex auf. Unsere Ergebnisse legen somit die Eignung des Markers DBC1 als immunhistochemische Routineuntersuchung für die Beurteilung der vom Resektionsstatus unabhängigen Prognose und Überlebenszeit von Ependymompatienten nahe.
Due to the wide variety of benign and malignant salivary gland tumors, classification and malignant behavior determination based on histomorphological criteria can be difficult and sometimes impossible. Spectroscopical procedures can acquire molecular biological information without destroying the tissue within the measurement processes. Since several tissue preparation procedures exist, our study investigated the impact of these preparations on the chemical composition of healthy and tumorous salivary gland tissue by Fourier-transform infrared (FTIR) microspectroscopy. Sequential tissue cross-sections were prepared from native, formalin-fixed and formalin-fixed paraffin-embedded (FFPE) tissue and analyzed. The FFPE cross-sections were dewaxed and remeasured. By using principal component analysis (PCA) combined with a discriminant analysis (DA), robust models for the distinction of sample preparations were built individually for each parotid tissue type. As a result, the PCA-DA model evaluation showed a high similarity between native and formalin-fixed tissues based on their chemical composition. Thus, formalin-fixed tissues are highly representative of the native samples and facilitate a transfer from scientific laboratory analysis into the clinical routine due to their robust nature. Furthermore, the dewaxing of the cross-sections entails the loss of molecular information. Our study successfully demonstrated how FTIR microspectroscopy can be used as a powerful tool within existing clinical workflows.
SUMMARY GABP is a heterodymeric member of Ets-family transcription factors. It consists of two subunits – GABPa which contains DNA binding domain and GABPb, which provides transcriptional activation domain and nuclear localization signal. GABPa/b complex is essential for transcriptional activation of multiple lineage-restricted and housekeeping genes, several viral genes, and in some cases might function as transcriptional repressor. Large variety of data indicates involvement of GABP in the complex regulation of cell growth, specified by quiescence, stimulation/proliferation, apoptosis and senescence. Expression level of GABPa subunit is rapidly increased when resting cells enter S-phase, and GABPa/b complex is critical to promote the continuity of the cell cycle. Conditional inactivation of GABPa expression in mouse embryonic fibroblasts results in a complete block of proliferation and acquisition of senescence-like phenotype. However, the influence of GABP on the other cell growth determinant – the apoptosis – remains largely obscure. Therefore we aimed to investigate the influence of GABPa/b expression level on the cell growth in vitro. Using siRNA approach we achieved efficient but only transient down-regulation of GABPa expression which precluded further cell growth studies. Persistent increase of the expression of GABPb subunit only resulted in a positive effect on the cell growth speed. Simultaneous conditional overexpression of both GABPa and GABPb subunits though, strongly reduced the growth of the affected cell cultures in reversible and in expression level dependent manner. Interestingly, GABPa/b overexpressing cells did show neither cell cycle arrest nor massive induction of apoptosis. However, more detailed analyses revealed that dampened apoptotic processes were taking place in GABPa/b−overexpressing cells, starting with a prominent activation of caspase-12. Interestingly, activation of downstream effector caspases was rather suppressed explaining a weak increase of apoptotic cells in GABPa/b overexpressing cultures. This effect suggests that the activation of caspase-12 by elevated amounts of exogenous GABPa/b reflects the normal physiological mechanism of caspase-12 regulation.
The transcriptional coactivator Bob1 promotes the development of follicular T helper cells via Bcl6

Follicular T helper (Tfh) cells are key regulators of the germinal center reaction and long-term humoral immunity. Tfh cell differentiation requires the sustained expression of the transcriptional repressor Bcl6; however, its regulation in CD4\(^+\) T cells is incompletely understood. Here, we report that the transcriptional coactivator Bob1, encoded by the Pou2af1 gene, promotes Bcl6 expression and Tfh cell development. We found that Bob1 together with the octamer transcription factors Oct1/Oct2 can directly bind to and transactivate the Bcl6 and Btla promoters. Mixed bone marrow chimeras revealed that Bob1 is required for the expression of normal levels of Bcl6 and BTLA, thereby controlling the pool size and composition of the Tfh compartment in a T cell-intrinsic manner. Our data indicate that T cell-expressed Bob1 is directly involved in Tfh cell differentiation and required for mounting normal T cell-dependent B-cell responses.
B-cells of the rheumatoid synovial tissue are a constant part of and, in some histopathological subtypes, the dominant population of the inflammatory infiltrate, located in the region of tissue destruction. The pattern of B-cell distribution and the relationship to the corresponding antigen-presenting cells (follicular dendritic reticulum cells: FDCs) show a great variety. B-cells may exhibit (i) a follicular organization forming secondary follicles; (ii) follicle-like patterns with irregularly formed FDC networks, and (iii) a diffuse pattern of isolated FDCs. Molecular analysis of immunoglobulin VH and VL genes from human synovial B-cell hybridomas and synovial tissue demonstrates somatic mutations due to antigen activation. The FDC formations in the synovial tissue may therefore serve as an environment for B-cell maturation, which is involved in the generation of autoantibodies. An autoantibody is defined as "pathogenic" if it fulfills the Witebsky-Rose-Koch criteria for classical autoimmune diseases: definition of the autoantibody; induction of the disease by transfer of the autoantibody; and isolation of the autoantibody from the disease-specific lesion. B-cells from rheumatoid synovial tissue show specificity for FcIgG, type II collagen, COMP, sDNA, tetanus toxoid, mitochondrial antigens (M2), filaggrin and bacterial HSPs. The contributions of these antigens to the pathogenesis of RA are still hypothetical. A possible contribution could derive from crossreactivity and epitope mimicry: due to crossreaction, an antibody directed originally against a foreign infectious agent could react with epitopes from articular tissues, perpetuating the local inflammatory process. The characteristic distribution pattern, the localisation within the area of tissue destruction, the hypermutated IgVH and IgVL genes, and their exclusive function to recognize conformation-dependent antigens suggest a central role for B-cells in the inflammatory process of rheumatoid arthritis. Therefore, the analysis of synovial B-cell hybridomas and experimental expression of synovial IgVH and IgVL genes will help to characterise the antigens responsible for the pathogenesis of rheumatoid arthritis. In the present study 55 IgVH genes amplified from 3 different anatomical regions of a RA patient were analysed adding further information on synovial B-cell maturation and recirculation in RA. This analysis demonstrated somatically mutated IgVh genes in all different regions with amino acid deletions and mixed IgVh molecules, suggesting the existence of a novel pathway to generate (auto)antibody specificities. The comparison of amino acid sequences of amplified genes belonging to the VH1 family (with predominantly the same germline counterpart) exhibited a strong homology, indicating an apparently conserved mutational pattern. This suggests that the number of antigens activating B-cells in the different locations is restricted. The most striking result was the finding of clonally related sequences in different anatomical regions indicating a recirculation of activated B-cells between the different affected joints. Also in the present study a synovial B-cell hybridoma was analyzed for its specific recognition of cartilage antigens. A heptameric peptide of cartilage oligomeric protein (COMP) could be defined as the target structure. The IgVH-gene (IgHV4-59*01) of the IgG2l hybridoma has somatically mutated genes with high R/S values in the CDR regions (9:2). Thus, indicating that this hybridoma originates from a synovial B-cell which has been antigen activated/selected for its affinity. To analyse the presence of the clonotypic IgHV4-59*01 sequences in other cases of RA and osteoarthritis (OA) synovitis, primers specific for the CDR3 rearrangement of this hybridoma were used. The clonotypic and clone related sequences (98 per cent ± 1 per cent homology) could only be detected in synovitis of RA cases but not in OA cases indicating that this B-cell is specific to RA synovitis. The identified heptameric peptide of COMP was used in a peptide ELISA to analyse whether there is a specific binding in RA serum samples. Serum samples (IgG) from RA patients (n=22) showed a significant higher efficiency to the COMP heptamer than the OA sera (n=24) and the age matched healthy controls (n=20) (for both p<1x10-4, Students t-test). The specificity of this B-cell hybridoma may therefore be defined as RA specific. Since COMP is restricted to cartilage and tendons which are organs specifically affected in RA this COMP specific autoantibody represents the first organ specific autoantibody in RA. The IgG2 COMP specific autoantibody with somatically mutated IgVH genes is different from germline encoded, antigen clearing IgM autoantibodies and may therefore be directly involved as an "arthritogenic autoantibody" in cartilage and tendons destruction by complement activation.
Diffuse großzellige B-Zell Lymphome stellen weltweit die größte Gruppe maligner B-Zell Non-Hodgkin-Lymphome dar und umfassen eine biologisch, genetisch und klinisch heterogene Gruppe lymphoider Tumoren, die als Hauptmerkmal große transformierte B-Lymphozyten mit vesikulären Kernen und prominenten Nukleoli aufweisen. Nur ungefähr 40% der Patienten zeigen ein Ansprechen auf eine konventionelle Chemotherapie. Um von einer präziseren Prognose profitieren zu können, ist eine reproduzierbare Klassifizierung dieser „inhomogenen Entität“ nicht nur für die Betroffenen wünschenswert. Dadurch könnten Krankheitsverläufe genauer vorhergesagt und die Therapie optimiert werden. Während akute Leukämien häufig mit einer Translokation assoziiert sind, zeigt sich bei B-Zell-Lymphomen ein weitaus komplexeres Bild an zytogenetischen Aberrationen. Diese werden einerseits mit der Etablierung des malignen Phänotyps, andererseits mit der Tumorprogression und der klonalen Evolution eines Tumors in Verbindung gebracht. Obwohl die meisten Neoplasien rekurrente und Tumor-spezifische klonale chromosomale Aberrationen aufweisen, ist es noch nicht gelungen, durch Etablieren genetischer „Marker“ eine zuverlässige Aussage über Verlauf und Ansprechen und damit über die Prognose dieser Erkrankung zu machen. Solche Aussagen sind bis dato nur auf Basis allgemeiner klinischer Parameter wie dem Allgemeinzustand der Patienten und der Höhe der Lactat-Dehydrogenase (LDH) im Serum gesichert möglich, die neben anderen Parametern im „International Prognostic Index“ zusammengefasst sind. Zytogenetische, molekulargenetische und in den letzten Jahren auch Hochdurchsatz-Untersuchungen, wie z.B. die Mikroarray-Technologie, haben bereits zu einem besseren Verständnis dieser molekularen Unterschiede geführt. Die WHO-Klassifikation stellt im Hinblick auf die Definition ‚biologischer’ Tumorgruppen einen deutlichen Fortschritt dar. Diese neueren Untersuchungen zeigen auf der Basis des Genexpressionsprofils von diffusen großzelligen B-Zell-Lymphomen die mögliche Unterteilbarkeit dieser diagnostischen Kategorie anhand zweier unterschiedlicher Phänotypen: GC-DLBCL zeigen eine molekulare Signatur, die vergleichbar mit normalen Keimzentrumszellen ist. Die andere Gruppe (ABC-DLBCL bzw. non-GC-DLBCL) entsteht aus Zellen, die die Keimzentrumsreaktion bereits durchlaufen haben. GC- und ABC-DLBCL-Patienten weisen bei einer Anthracyclin-haltigen Chemotherapie (z. B. CHOP) in retrospektiven klinischen Studien einen deutlichen Unterschied in der 5-Jahres-Überlebensrate auf (etwa 60% für GC-DLBCL und 35% für ABC-DLBCL). Somit scheint die Annahme, dass mindestens zwei Entitäten vorliegen, gerechtfertigt. In der vorliegenden Studie konnten das zytogenetische Aberrationsspektrum mittels eines Algorithmus anhand des Genexpressionsprofiles mit zwei verschiedenen Subgruppen – den GC-DLBCL und den non-GC-DLBCL - korreliert werden. Es entstand die bisher umfangreichste zytogenetische Charakterisierung dieser postulierten Phänotypen. Es konnte gezeigt werden, dass GC-DLBCL, die durch die Translokation t(14;18) charakterisiert sind, häufiger Zugewinne bei Chromosom 7 aufweisen, während non-GC-DLBCL mit dem Vorliegen einer Trisomie 3 und Zugewinnen bei 3p und 3q assoziiert sind. Zwei Modelle könnten eine Erklärung für die Assoziation genomischer Instabilitäten mit den unterschiedlichen Genexpressionsprofilen sein. Einerseits könnte eine gewisse Region für die Kodierung eines Schlüsselregulatorgenes verantwortlich sein, welches die Zellbiologie und damit die Genexpression verändert. Andererseits könnten die Subgruppen der DLBCL auf verschiedenem Wege entstehen und somit unterschiedliche Ereignisse für die verschiedenen Aberrationen verantwortlich sein. Es gilt nun, einerseits diese Mechanismen, die zu einer veränderten Genexpression beitragen, aufzudecken und andererseits ein diagnostisches Vorgehen zu etablieren, bei dem beispielsweise nach dem erfolgten Nachweis von Index-Aberrationen eine bestimmte molekulare Signatur überprüft wird. Es ist anzunehmen, dass molekulare Analysen in absehbarer Zeit vermehrt Einzug in den klinischen Alltag halten und therapeutische Entscheidungen mit beeinflussen werden. In Zukunft wird anhand der Expression von Schlüsselgenen die Zuordnung eines Falles in diagnostische Untergruppen erfolgen. Außerdem könnte durch Peptidblockade dieser Schlüsselgene eine zusätzliche Therapieoption bestehen – eine Vermutung, die weitere Studien erforderlich macht.
