Linear, dimeric, tetrameric, and cyclic peptides derived from lactoferricin B, containing the RRWQWR motif, were designed, synthesized, purified, and characterized using RP-HPLC chromatography and MALDI-TOF mass spectrometry. The antibacterial activity of the designed peptides against E. coli (ATCC 11775 and 25922) and their cytotoxic effect against MDA-MB-468 and MDA-MB-231 breast cancer cell lines were evaluated. Dimeric and tetrameric peptides showed higher antibacterial activity in both bacteria strains than linear peptides. The dimeric peptide (RRWQWR)\(_2\)K-Ahx exhibited the highest antibacterial activity against the tested bacterial strains. Furthermore, the peptides with high antibacterial activity exhibited significant cytotoxic effect against the tested breast cancer cell lines. This cytotoxic effect was fast and dependent on the peptide concentration. The tetrameric molecule containing RRWQWR motif has an optimal cytotoxic effect at a concentration of 22 µM. The evaluated dimeric and tetrameric peptides could be considered as candidates for developing new therapeutic agents against breast cancer. Polyvalence of linear sequences could be considered as a novel and versatile strategy for obtaining molecules with high anticancer activity.
Peptides derived from human and bovine lactoferricin were designed, synthesized, purified, and characterized using RP-HPLC and MALDI-TOF-MS. Specific changes in the sequences were designed as (i) the incorporation of unnatural amino acids in the sequence, the (ii) reduction or (iii) elongation of the peptide chain length, and (iv) synthesis of molecules with different number of branches containing the same sequence. For each peptide, the antibacterial activity against Escherichia coli ATCC 25922 and Enterococcus faecalis ATCC 29212 was evaluated. Our results showed that Peptides I.2 (RWQWRWQWR) and I.4 ((RRWQWR)\(_{4}\)K\(_{2}\)Ahx\(_{2}\)C\(_{2}\)) exhibit bigger or similar activity against E. coli (MIC 4-33 μM) and E. faecalis (MIC 10-33 μM) when they were compared with lactoferricin protein (LF) and some of its derivate peptides as II.1 (FKCRRWQWRMKKLGA) and IV.1 (FKCRRWQWRMKKLGAPSITCVRRAE). It should be pointed out that Peptides I.2 and I.4, containing the RWQWR motif, are short and easy to synthesize; our results demonstrate that it is possible to design and obtain synthetic peptides that exhibit enhanced antibacterial activity using a methodology that is fast and low-cost and that allows obtaining products with a high degree of purity and high yield.
Mit Hilfe der Einzelzell PCR wurden CD 19+ B-Zellen aus dem peripheren Blut von zwei Patienten mit systemischen Lupus erythematodes isoliert und anschließend die Kappa-Leichtketten Rearrangements analysiert. Es konnten 194 nichtproduktiv und 221 produktiv rearrangierte Immunblobuline sequenziert werden und mit alterstypischen Repertoires verglichen werden. Dazu zählten die B-Zellen aus dem Nabelschnurblut, verschiedene B-Zellen der kindlichen Tonsille und B-Zellen aus dem Erwachsenenblut. Die Schwerpunkt der Analyse lagen dabei auf dem Aufbau des Kappa-Repertoire aus den einzelnen V-Kappa-Familien, auf der Modifikation der VkJk-Schnittstelle durch die verschiedenen Rekombiantionsenzyme (TdT, Exonukleasen, durch P- und N-Nukleotide sowie auf der Modifikation der Immunglobuline durch somatische Hypermutation.
Analyse der ontogenetischen Veränderungen in B-Zell-Subpopulationen im Kindes- und Erwachsenenalter

B-Lymphozyten sind die zellulären Träger der humoralen Immunität des adaptiven Immunsystems. Sie spielen eine wesentliche Rolle bei Immundefekten und Autoimmunprozessen. In dieser Arbeit sollen Entwicklungsveränderungen der peripheren B-Zell-Populationen von der Kindheit bis zum Erwachsenenalter charakterisiert und altersabhängige Referenzwerte generiert werden. In einer durchflusszytometrischen Analyse wurden dafür sowohl relative als auch absolute Häufigkeiten für naive B-Zellen, Gedächtnis-B-Zellen, Transitionalzellen, Plasmablasten und CD21 low CD38 low B-Zellen untersucht. Die meisten B-Zell-Subpopulationen zeigen spezifische ontogenetische Veränderungen.
Analyse der Immunglobulinrepertoire-Veränderungen in B Zellen von Kindern mit Autoimmunerkrankungen

B Zellen sind die zellulären Träger einer gegen Infektionserreger gerichteten, humoralen und protektiven Immunität. In der Entstehung von Autoimmunerkrankungen und in der Aufrechterhaltung von chronischen Entzündungen scheint diesen Zellen eine entscheidende Rolle zuzukommen. In dieser Arbeit wurden in B Zellen von Patienten mit rheumatischen Erkrankungen molekulare Mechanismen untersucht, die es der B Zelle außerhalb des Knochenmarkes erlauben, ihre Rezeptorspezifität zu verändern. Ein solcher Vorgang könnte Verschiebungen des Immunglobulinrepertoires in Richtung Autoreaktivität erklären.
Die Immunthrombozytopenie (ITP) ist eine erworbene Autoimmunerkrankung, bei der sich Autoantikörper gegen Thrombozyten bilden. Dadurch werden diese, unter anderem in der Milz, vermehrt abgebaut und es treten Blutungskomplikationen auf. Der fehlgeleiteten Immunabwehr wird versucht mit medikamentösen Therapien wie z. B. mit Glucocorticoiden und Rituximab bis hin zur Splenektomie entgegenzuwirken.
Im Rahmen dieser Arbeit untersuchte ich durchflusszytometrisch die Verteilung der B-Zell-Subpopulationen bei Patienten mit chronischer, primärer ITP und Gesunden hinsichtlich einer möglichen Störung in der B-Zell-Entwicklung und -Differenzierung. Dabei wurden 7 Knochenmark-, 28 Blut- und 12 Milzproben von ITP-Patienten sowie 5 Knochenmark- und 10 Milzproben von Gesunden aufbereitet. Anschließend wurden die B-Zell-Subpopulationen mittels immunphänotypischer Marker gefärbt um die Proben danach durchflusszytometrisch zu vermessen und zu charakterisieren. Zusätzlich erfolgte der Vergleich zu laboreigenen, bereits etablierten Referenzwerten von 220 Blutproben von Gesunden.
Bei den Knochenmarkproben konnten keine signifikanten Unterschiede in der Verteilung von Vorläufer-B-Zellen zwischen den ITP-Patienten und den Gesunden beobachtet werden, d. h. die frühe B-Zell-Entwicklung im Knochenmark erscheint bei der ITP auf zellulärer Ebene nicht beeinträchtigt. Die Analyse der Blutproben zeigte, dass auch keine signifikanten Unterschiede in der Verteilung von naiven B-Zellen zwischen den ITP-Patienten und den Gesunden vorzufinden sind. Dies bekräftigt, dass bei der ITP auf zellulärer Ebene keine Abweichungen in der frühen pre-immunen B-Zell-Entwicklung vorzuliegen scheinen und eine intakte B-Zell-Reifung bis hin zur naiven B-Zelle stattfindet. Es zeigte sich jedoch bei den ITP-Patienten ein erhöhter Anteil an anergen B-Zellen und atypischen Gedächtnis-B-Zellen, von denen allgemein angenommen wird, dass sie aus einer chronischen bzw. dysregulierten antigen-abhängigen B-Zell-Aktivierung entstammen. Aus der Untersuchung der Milzproben zeigte sich zudem, dass bei den ITP-Patienten der Anteil der antikörperproduzierenden Plasmablasten im Vergleich zu den Gesunden erhöht ist. Folglich lassen sich bei der ITP auf zellulärer Ebene vor allem Abweichungen in der späten Phase der B-Zell-Differenzierung nachweisen. Es kann somit angenommen werden, dass Störungen der B-Zell-Entwicklung, wie sie auf zellulärer Ebene bei verschiedenen mit sekundärer ITP einhergehenden Erkrankungen (systemischer Lupus erythematodes, variables Immundefektsyndrom) beschrieben wurden, bei der primären ITP nicht für die Produktion von antithrombozytären Antikörpern notwendig sind. Eine weitere detaillierte Aufarbeitung, auf welcher Ebene der B-Zell-Differenzierung der Toleranzverlust gegenüber thrombozytären Antigenen auftritt, ist entscheidend für die zukünftige Entwicklung spezifischer, zellgerichteter Therapien.
Einleitung: Im Verlauf der letzten Jahre hat sich die Messung akustisch evozierter Hirnstammpotentiale als Untersuchungsmethode in der Pädiatrie, Hals-Nasen-Ohren-Heilkunde und Neurologie etabliert. Ziel dieser Studie war es Latenzwerte der Welle I, III und V von Frühgeborenen unterschiedlicher Gestationswochen (GW) zu erfassen. Es sollte überprüft werden, ob es im Frühgeborenenalter allgemein zu einer kontinuierlichen Verkürzung der Latenzen aller drei Wellen (I, III, V) kommt oder ob Schwankungen im Verlauf bestehen, um somit bei späteren Untersuchungen Voraussagen über Latenzwerte machen zu können. Die Latenzwerte sollten sowohl in der Zeitgang-, als auch in der Standard-BERA gemessen und miteinander verglichen werden. Ebenso sollte die Abhängigkeit der Latenzen der AEHP von Risikofaktoren, hier Gentamicintherapie, im Patientenkollektiv dargestellt und die beiden verwendeten Meßmethoden miteinander verglichen werden. Methoden: Die Messung der Hirnstammpotentiale erfolgte mittels des BERAphon der Firma WESTRA, welches sich aus einem Computer und einen Kopfhörer mit drei Dauerelektroden zusammensetzt, die eine Vereinfachung der Handhabung des Gerätes erlauben. Insgesamt wurden 118 Messungen an 56 Neugeborenen, mit einem Gestationsalter von 23-40 Wochen zum Zeitpunkt der Geburt, durchgeführt. Das Alter der Patienten wurde zum Zeitpunkt der Messung auf die jeweils korrigierte Schwangerschaftswoche erhöht und lag damit zwischen der 30. und 49. Woche. Das Patientenkollektiv stammte aus der Kinderklinik der Universität Würzburg. Je nach Aufenthaltsdauer in der Klinik fanden auch longitudinale Messungen statt. Die Latenzen der Welle I, III und V wurden bei 40dB, 50dB und 60dB im Zeitgang- und Standard-Verfahren bestimmt. Gemessen wurden jeweils beide Ohren. Dabei betrug die Messdauer insgesamt 30 Minuten bis eine Stunde. Ergebnisse: Ergebnis dieser Studie war, dass es im Verlauf der Frühgeborenenperiode zu einer Latenzverkürzung kommt. Das Maximum der Latenzverkürzung konnte in dieser Studie zwischen der 31. und 34. GW beobachtet werden. Demnach scheint es insbesondere in dieser Entwicklungsperiode zu einer Reifung des auditiven Systems zu kommen. Bei Betrachtung des Einflusses von Gentamicin zeigte sich, dass die Patienten zwar anfangs höhere Latenzmaxima aufwiesen, sich jedoch im Verlauf bis zur 39. GW den Latenzwerten des Vergleichskollektives ohne Gentamicintherapie anglichen. Die Ursache hierfür scheint eine reversible Störung der Hörfunktion unter dieser Medikation zu sein. Beim Vergleich der Zeitgang- und Standard-BERA konnte beobachtet werden, dass die Standard-BERA im Allgemeinen kürzere Latenzwerte als das Zeitgangverfahren aufweist, sich die Werte beider Verfahren jedoch bei geringeren GW nahezu angleichen und bei einer Reizstärke von 40dB keines der beiden Verfahren bezüglich kürzerer Latenzen dominiert. Bei 50dB hingegen bietet das Zeitgangverfahren deutlich schlechtere Latenzen als das Standardverfahren. Es scheint demnach ein „Schwellenwert“ notwendig zu sein, um eine ausreichende Verschaltung im Hirnstamm zu erreichen. Die 40dB-Stufe ist möglicherweise als Reiz ungenügend, sodass in beiden Verfahren Latenzen mit großer Streubreite auftreten. Dies erklärt die größere Übereinstimmung beider Verfahren bei höheren Reizstufen. Bei Betrachtung der Wellen I, III und V stellte sich heraus, dass alle drei Wellen unterschiedlich gut darstellbar waren. Es zeigte sich, dass Welle I am schlechtesten, Welle V hingegen am besten im Kollektiv der Frühgeborenen darstellbar und in jedem Entwicklungsalter zu finden war. In jedem Entwicklungsalter stellten sich spezifische Latenzen dar. Wellen I und III korrelierten signifikant häufiger in beiden Verfahren bei steigendem Reizpegel miteinander und Welle V zeigte die höchste Übereinstimmung der Messwerte in beiden Verfahren. Schlussfolgerung: Frühgeborenen zeigen demnach im Verlauf ihrer Entwicklung, selbst unter Therapie mit potentiell ototoxischer Medikation, hier Gentamicin, und insbesondere zwischen der 31. und 34. GW eine Reifung des auditiven Systems, erkennbar an einer Latenzverkürzung der AEHP. Prinzipiell sollte bei allen Neugeboren vor Entlassung aus der Klinik ein Hörscreening durchgeführt werden, um eine Hörstörung möglichst früh aufzudecken. Zu empfehlen ist hierfür die Zeitgang-BERA, da sie mit wenig Zeitaufwand einen Überblick über die Hörschwelle in einem Untersuchungsgang erlaubt. Bei Kindern mit komplexeren Erkrankungen oder jüngerer GW sollte jedoch bei noch ungenügender Reife des auditiven Systems und zur weiteren Beurteilbarkeit des Hirnstammes die Standard-BERA durchgeführt werden. Risiko-Neugeborene und damit insbesondere Frühgeborene sollten jedoch zur weiteren Verlaufkontrolle einem zweiten Screening nach 3-4 Monaten unterzogen werden.
Adrenocortical tumors are rare in children. This systematic review summarizes the published evidence on pediatric adrenocortical carcinoma (ACC) to provide a basis for a better understanding of the disease, investigate new molecular biomarkers and therapeutic targets, and define which patients may benefit from a more aggressive therapeutic approach. We included 137 studies with 3680 ACC patients (~65% female) in our analysis. We found no randomized controlled trials, so this review mainly reflects retrospective data. Due to a specific mutation in the TP53 gene in ~80% of Brazilian patients, that cohort was analyzed separately from series from other countries. Hormone analysis was described in 2569 of the 2874 patients (89%). Most patients were diagnosed with localized disease, whereas 23% had metastasis at primary diagnosis. Only 72% of the patients achieved complete resection. In 334 children (23%), recurrent disease was reported: 81% — local recurrence, 19% (n = 65) — distant metastases at relapse. Patients < 4 years old had a different distribution of tumor stages and hormone activity and better overall survival (p < 0.001). Although therapeutic approaches are typically multimodal, no consensus is available on effective standard treatments for advanced ACC. Thus, knowledge regarding pediatric ACC is still scarce and international prospective studies are needed to implement standardized clinical stratifications and risk-adapted therapeutic strategies.
Background
Adrenalectomies are rare procedures especially in childhood. So far, no large cohort study on this topic has been published with data on to age distribution, operative procedures, hospital volume and operative outcome.
Methods
This is a retrospective analysis of anonymized nationwide hospital billing data (DRG data, 2009-2017). All adrenal surgeries (defined by OPS codes) of patients between the age 0 and 21 years in Germany were included.
Results
A total of 523 patient records were identified. The mean age was 8.6 ± 7.7 years and 262 patients were female (50.1%). The majority of patients were between 0 and 5 years old (52% overall), while 11.1% were between 6 and 11 and 38.8% older than 12 years. The most common diagnoses were malignant neoplasms of the adrenal gland (56%, mostly neuroblastoma) with the majority being younger than 5 years. Benign neoplasms in the adrenal gland (D350) account for 29% of all cases with the majority of affected patients being 12 years or older. 15% were not defined regarding tumor behavior. Overall complication rate was 27% with a clear higher complication rate in resection for malignant neoplasia of the adrenal gland. Bleeding occurrence and transfusions are the main complications, followed by the necessary of relaparotomy. There was an uneven patient distribution between hospital tertiles (low volume, medium and high volume tertile). While 164 patients received surgery in 85 different “low volume” hospitals (0.2 cases per hospital per year), 205 patients received surgery in 8 different “high volume” hospitals (2.8 cases per hospital per year; p<0.001). Patients in high volume centers were significant younger, had more extended resections and more often malignant neoplasia. In multivariable analysis younger age, extended resections and open procedures were independent predictors for occurrence of postoperative complications.
Conclusion
Overall complication rate of adrenalectomies in the pediatric population in Germany is low, demonstrating good therapeutic quality. Our analysis revealed a very uneven distribution of patient volume among hospitals.
Etwa 50 % der Kinder und Jugendlichen mit Kraniopharyngeom entwickeln im Verlauf eine ausgeprägte Adipositas, welche ein therapeutisch nur schwer beeinflussbares Problem darstellt. In der vorliegenden Arbeit wurden Risikofaktoren für die Entwicklung von Adipositas nach Kraniopharyngeomtherapie ermittelt mit dem Ziel, präventive Maßnahmen gezielter und frühzeitiger einsetzen zu können. In dieser multizentrischen Querschnittsuntersuchung wurden 203 Patienten nachuntersucht, bei denen im Kindes- und Jugendalter die Diagnose eines Kraniopharyngeoms gestellt wurde. Um Risikofaktoren für eine Adipositas bei Kraniopharyngeompatienten zu ermitteln, wurden die normalgewichtigen mit den adipösen Patienten verglichen. Die folgenden signifikanten Unterschiede wurden ermittelt: Adipöse Patienten hatten größere Tumore, die in der Bildgebung häufiger Hypothalamusbeteiligung und Verkalkungen aufzeigten und häufiger zu erweiterten Seitenventrikeln führten. Bei den Patienten, die eine Adipositas entwickelten, war retrospektiv häufiger schon vor der Diagnosestellung eine Gewichtszunahme aufgefallen als bei normalgewichtigen Patienten. Ein Wachstumsknick war anamnestisch bei adipösen Patienten seltener aufgefallen. Tatsächlich waren die adipösen Patienten zum Zeitpunkt der Diagnosestellung schwerer und größer als die normalgewichtigen Patienten. Wir folgern aus diesen Ergebnissen, dass Risikofaktoren für eine spätere Adipositas bei Kraniopharyngeompatienten bereits präoperativ ermittelt werden können. Deshalb könnte der frühzeitige Einsatz von präventiven und therapeutischen Maßnahmen die spätere Lebensqualität der Patienten verbessern.
Cystic fibrosis (CF) is one of the most common inherited diseases and is caused by mutations in the CFTR gene. Although the pulmonary and gastrointestinal manifestations of the disease remain in the focus of treatment, recent studies have shown expression of the CFTR gene product in skeletal muscle cells and observed altered intramuscular \(Ca^{2+}\) release dynamics in CFTR-deficient animal models. Physical exercise is beneficial for maintaining fitness and well-being in CF patients and constitutes one aspect of modern multimodal treatment, which has considerably increased life span and reduced morbidity. We report on a case of acute muscle trauma resulting from excessive dumbbell exercise in a young adult with cystic fibrosis and describe clinical, laboratory and imaging characteristics of acute exercise-induced muscle injury.
Acellular Pertussis Booster in Adolescents Induces Th1 and Memory CD8+ T Cell Immune Response

In a number of countries, whole cell pertussis vaccines (wcP) were replaced by acellular vaccines (aP) due to an improved reactogenicity profile. Pertussis immunization leads to specific antibody production with the help of CD4+ T cells. In earlier studies in infants and young children, wcP vaccines selectively induced a Th1 dominated immune response, whereas aP vaccines led to a Th2 biased response. To obtain data on Th1 or Th2 dominance of the immune response in adolescents receiving an aP booster immunization after a wcP or aP primary immunization, we analyzed the concentration of Th1 (IL-2, TNF-a, INF-c) and Th2 (IL-4, IL-5, IL-10) cytokines in supernatants of lymphocyte cultures specifically stimulated with pertussis antigens. We also investigated the presence of cytotoxic T cell responses against the facultative intracellular bacterium Bordetella pertussis by quantifying pertussis-specific CD8+ T cell activation following the aP booster immunization. Here we show that the adolescent aP booster vaccination predominantly leads to a Th1 immune response based on IFNgamma secretion upon stimulation with pertussis antigen, irrespective of a prior whole cell or acellular primary vaccination. The vaccination also induces an increase in peripheral CD8+CD69+ activated pertussis-specific memory T cells four weeks after vaccination. The Th1 bias of this immune response could play a role for the decreased local reactogenicity of this adolescent aP booster immunization when compared to the preceding childhood acellular pertussis booster. Pertussis-specific CD8+ memory T cells may contribute to protection against clinical pertussis.
Chorioamnionitis is associated with an increased risk of preterm birth and aggravates adverse outcomes such as BPD. Development of BPD is associated with chronic inflammatory reactions and oxidative stress in the airways which may be antenatally initiated by chorioamnionitis. A20 is an immunomodulatory protein involved in the negative feedback regulation of inflammatory reactions and is a possible regulator protein in oxidative stress reactions. The influence of chorioamnionitis on A20 gene regulation in the fetal lung is unknown. We characterized the influence of LPS and proinflammatory cytokines on A20 expression in human lung endothelial (HPMEC-ST1.6R) and epithelial (A549) cells in vitro by real-time PCR and/or western blotting and used a sheep model of LPS-induced chorioamnionitis for in vivo studies. To study the functional role of A20, endogenous A20 was overexpressed in HPMEC-ST1.6R and A549 cells. LPS induced proinflammatory cytokines in HPMEC-ST1.6R and A549 cells. Both LPS and/or proinflammatory cytokines elevated A20 at transcriptional and translational levels. Intra-amniotic LPS transiently increased IL-1β, IL-6, IL-8, and TNF-α mRNA levels in fetal lamb lungs, associated with an increase in A20 mRNA and protein levels. Overexpression of A20 reduced proinflammatory cytokines in vitro. Repeated LPS exposure induced LPS tolerance for proinflammatory cytokines and A20 in vitro and in vivo. Antenatal inflammation induced a transient increase in proinflammatory cytokines in the preterm fetal lung. The expression of proinflammatory cytokines increased expression of A20. Elevated A20 may have a protective role by downregulating chorioamnionitis-triggered fetal lung inflammation. A20 may be a novel target for pharmacological interventions to prevent chorioamnionitis-induced airway inflammation and lung damage, which can result in BPD later in life.
Acute graft-versus-host disease (aGvHD) is a severe and often life-threatening complication of allogeneic hematopoietic cell transplantation (allo-HCT). AGvHD is mediated by alloreactive donor T-cells targeting predominantly the gastrointestinal tract, liver, and skin. Recent work in mice and patients undergoing allo-HCT showed that alloreactive T-cells can be identified by the expression of α4β7 integrin on T-cells even before manifestation of an aGvHD. Here, we investigated whether the detection of a combination of the expression of T-cell surface markers on peripheral blood (PB) CD8\(^+\) T-cells would improve the ability to predict aGvHD. To this end, we employed two independent preclinical models of minor histocompatibility antigen mismatched allo-HCT following myeloablative conditioning. Expression profiles of integrins, selectins, chemokine receptors, and activation markers of PB donor T-cells were measured with multiparameter flow cytometry at multiple time points before the onset of clinical aGvHD symptoms. In both allo-HCT models, we demonstrated a significant upregulation of α4β7 integrin, CD162E, CD162P, and conversely, a downregulation of CD62L on donor T-cells, which could be correlated with the development of aGvHD. Other surface markers, such as CD25, CD69, and CC-chemokine receptors were not found to be predictive markers. Based on these preclinical data from mouse models, we propose a surface marker panel on peripheral blood T-cells after allo-HCT combining α4β7 integrin with CD62L, CD162E, and CD162P (cutaneous lymphocyte antigens, CLA, in humans) to identify patients at risk for developing aGvHD early after allo-HCT.
Background: Beyond survival of nowadays >80%, modern childhood cancer treatment strives to preserve long-term health and quality of life. However, the majority of today’s survivors suffer from short- and long-term adverse effects such as cardiovascular and pulmonary diseases, obesity, osteoporosis, fatigue, depression, and reduced physical fitness and quality of life. Regular exercise can play a major role to mitigate or prevent such late-effects. Despite this, there are no data on the effects of regular exercise in childhood cancer survivors from randomized controlled trials (RCTs). \(Primary\) \(outcome\) of the current RCT is therefore the effect of a 12-months exercise program on a composite cardiovascular disease risk score in childhood cancer survivors. \(Secondary\) \(outcomes\) are single cardiovascular disease risk factors, glycaemic control, bone health, body composition, physical fitness, physical activity, quality of life, mental health, fatigue and adverse events (safety).
Methods: A total of 150 childhood cancer survivors aged ≥16 years and diagnosed ≥5 years prior to the study are recruited from Swiss paediatric oncology clinics. Following the baseline assessments patients are randomized 1:1 into an intervention and control group. Thereafter, they are seen at month 3, 6 and 12 for follow-up assessments. The intervention group is asked to add ≥2.5 h of intense physical activity/week, including 30 min of strength building and 2 h of aerobic exercises. In addition, they are told to reduce screen time by 25%. Regular consulting by physiotherapists, individual web-based activity diaries, and pedometer devices are used as motivational tools for the intervention group. The control group is asked to keep their physical activity levels constant.
Discussion: The results of this study will show whether a partially supervised exercise intervention can improve cardiovascular disease risk factors, bone health, body composition, physical activity and fitness, fatigue, mental health and quality of life in childhood cancer survivors. If the program will be effective, all relevant information of the SURfit physical activity intervention will be made available to interested clinics that treat and follow-up childhood cancer patients to promote exercise in their patients.
Hyper-IgM syndrome type 2 (HIGM2) is a B cell intrinsic primary immunodeficiency caused by mutations in AICDA encoding activation-induced cytidine deaminase (AID) which impair immunoglobulin class switch recombination (CSR) and somatic hypermutation (SHM). Whereas autosomal-recessive AID-deficiency (AR-AID) affects both CSR and SHM, the autosomal-dominant form (AD-AID) due to C-terminal heterozygous variants completely abolishes CSR but only partially affects SHM. AR-AID patients display enhanced germinal center (GC) reactions and autoimmune manifestations, which are not present in AD-AID, suggesting that SHM but not CSR regulates GC reactions and peripheral B cell tolerance. Herein, we describe two siblings with HIGM2 due to a novel homozygous AICDA mutation (c.428-1G > T) which disrupts the splice acceptor site of exon 4 and results in the sole expression of a truncated AID variant that lacks 10 highly conserved amino acids encoded by exon 4 (AID-ΔE4a). AID-ΔE4a patients suffered from defective CSR and enhanced GC reactions and were therefore indistinguishable from other AR-AID patients. However, the AID-ΔE4a variant only partially affected SHM as observed in AD-AID patients. In addition, AID-ΔE4a but not AD-AID patients revealed impaired targeting of mutational hotspot motives and distorted mutational patterns. Hence, qualitative defects in AID function and altered SHM rather than global decreased SHM activity may account for the disease phenotype in these patients.
Background
Lipoblastoma is a rare benign mesenchymal neoplasm of infancy that most commonly occurs on the extremities and trunk but can arise at variable sites of the body. Retroperitoneal lipoblastomas are particularly rare but can grow to enormous size, and preoperative diagnosis is difficult with diverse, mostly malignant differential diagnoses that would lead to aggressive therapy. Since lipoblastoma is a benign tumor that has an excellent prognosis after resection, correct diagnosis is crucial.
Case presentation
A case of a large retroperitoneal tumor of a 24-month old infant that was clinically suspicious of a malignant tumor is presented. Due to proximity to the right kidney, clinically most probably a nephroblastoma or clear cell sarcoma of the kidney was suspected. Radiological findings were ambiguous. Therefore, the mass was biopsied, and histology revealed an adipocytic lesion. Although mostly composed of mature adipocytes, in view of the age of the patient, the differential diagnosis of a (maturing) lipoblastoma was raised, which was supported by molecular analysis demonstrating a HAS2-PLAG1 fusion. The tumor was completely resected, and further histopathological workup led to the final diagnosis of a 13 cm large retroperitoneal maturing lipoblastoma. The child recovered promptly from surgery and showed no evidence of recurrence so far.
Conclusion
Although rare, lipoblastoma should be included in the differential diagnoses of retroperitoneal tumors in infants and children, and molecular diagnostic approaches could be a helpful diagnostic adjunct in challenging cases.
Background
Acute graft-versus-host disease (aGVHD) poses a major limitation for broader therapeutic application of allogeneic hematopoietic cell transplantation (allo-HCT). Early diagnosis of aGVHD remains difficult and is based on clinical symptoms and histopathological evaluation of tissue biopsies. Thus, current aGVHD diagnosis is limited to patients with established disease manifestation. Therefore, for improved disease prevention it is important to develop predictive assays to identify patients at risk of developing aGVHD. Here we address whether insights into the timing of the aGVHD initiation and effector phases could allow for the detection of migrating alloreactive T cells before clinical aGVHD onset to permit for efficient therapeutic intervention.
Methods
Murine major histocompatibility complex (MHC) mismatched and minor histocompatibility antigen (miHAg) mismatched allo-HCT models were employed to assess the spatiotemporal distribution of donor T cells with flow cytometry and in vivo bioluminescence imaging (BLI). Daily flow cytometry analysis of peripheral blood mononuclear cells allowed us to identify migrating alloreactive T cells based on homing receptor expression profiles.
Results
We identified a time period of 2 weeks of massive alloreactive donor T cell migration in the blood after miHAg mismatch allo-HCT before clinical aGVHD symptoms appeared. Alloreactive T cells upregulated α4β7 integrin and P-selectin ligand during this migration phase. Consequently, targeted preemptive treatment with rapamycin, starting at the earliest detection time of alloreactive donor T cells in the peripheral blood, prevented lethal aGVHD.
Conclusions
Based on this data we propose a critical time frame prior to the onset of aGVHD symptoms to identify alloreactive T cells in the peripheral blood for timely and effective therapeutic intervention.
Background
Mucopolysaccharidosis type III (Sanfilippo syndrome) is a lysosomal storage disorder, caused by a deficiency in the heparan-N-sulfatase enzyme involved in the catabolism of the glycosaminoglycan heparan sulfate. It is characterized by early nonspecific neuropsychiatric symptoms, followed by progressive neurocognitive impairment in combination with only mild somatic features. In this patient group with a broad clinical spectrum a significant genotype-phenotype correlation with some mutations leading to a slower progressive, attenuated course has been demonstrated.
Case presentation
Our patient had complications in the neonatal period and was diagnosed with Mucopolysaccharidosis IIIa only at the age of 28 years. He was compound heterozygous for the variants p.R245H and p.S298P, the latter having been shown to lead to a significantly milder phenotype.
Conclusions
The diagnostic delay is even more prolonged in this patient population with comorbidities and a slowly progressive course of the disease.
