Background: Large Cell Neuroendocrine Carcinoma (LCNEC) is a rare subtype of lung cancer with poor clinical outcomes. Data on recurrence-free survival (RFS) in early and locally advanced pure LCNEC after complete resection (R0) are lacking. This study aims to evaluate clinical outcomes in this subgroup of patients and to identify potential prognostic markers. Methods: Retrospective multicenter study including patients with pure LCNEC stage I-III and R0 resection. Clinicopathological characteristics, RFS, and disease-specific survival (DSS) were evaluated. Univariate and multivariate analyses were performed. Results: 39 patients (M:F = 26:13), with a median age of 64 years (44–83), were included. Lobectomy (69.2%), bilobectomy (5.1%), pneumonectomy (18%), and wedge resection (7.7%) were performed mostly associated with lymphadenectomy. Adjuvant therapy included platinum-based chemotherapy and/or radiotherapy in 58.9% of cases. After a median follow-up of 44 (4–169) months, the median RFS was 39 months with 1-, 2- and 5-year RFS rates of 60.0%, 54.6%, and 44.9%, respectively. Median DSS was 72 months with a 1-, 2- and 5-year rate of 86.8, 75.9, and 57.4%, respectively. At multivariate analysis, age (cut-off 65 years old) and pN status were independent prognostic factors for both RFS (HR = 4.19, 95%CI = 1.46–12.07, p = 0.008 and HR = 13.56, 95%CI 2.45–74.89, p = 0.003, respectively) and DSS (HR = 9.30, 95%CI 2.23–38.83, p = 0.002 and HR = 11.88, 95%CI 2.28–61.84, p = 0.003, respectively). Conclusion: After R0 resection of LCNEC, half of the patients recurred mostly within the first two years of follow-up. Age and lymph node metastasis could help to stratify patients for adjuvant therapy.
In Eurotransplant kidney allocation system (ETKAS), candidates can be considered unlimitedly for repeated re‐transplantation. Data on outcome and benefit are indeterminate. We performed a retrospective 15‐year patient and graft outcome data analysis from 1464 recipients of a third or fourth or higher sequential deceased donor renal transplantation (DDRT) from 42 transplant centers. Repeated re‐DDRT recipients were younger (mean 43.0 vs. 50.2 years) compared to first DDRT recipients. They received grafts with more favorable HLA matches (89.0% vs. 84.5%) but thereby no statistically significant improvement of patient and graft outcome was found as comparatively demonstrated in 1st DDRT. In the multivariate modeling accounting for confounding factors, mortality and graft loss after 3rd and ≥4th DDRT (P < 0.001 each) and death with functioning graft (DwFG) after 3rd DDRT (P = 0.001) were higher as compared to 1st DDRT. The incidence of primary nonfunction (PNF) was also significantly higher in re‐DDRT (12.7%) than in 1st DDRT (7.1%; P < 0.001). Facing organ shortage, increasing waiting time, and considerable mortality on dialysis, we question the current policy of repeated re‐DDRT. The data from this survey propose better HLA matching in first DDRT and second DDRT and careful selection of candidates, especially for ≥4th DDRT.
Merkel cell carcinoma (MCC) is an aggressive, virus-associated, neuroendocrine tumor of the skin mainly affecting immunocompromised patients. Higher intratumoral infiltration with CD3 and CD8 positive T-cells is associated with a better prognosis, highlighting the relevance of the immune system for MCC development and progression. In this study 21 primary MCCs were stained with immune cell markers including CD3, CD4, CD8, CD68, CD20, and S100. Furthermore, tumor-infiltrating neutrophils, tertiary lymphoid structures and PD-L1 expression were analyzed and correlated with overall and recurrence free survival. All MCCs were Merkel Cell Polyomavirus positive. Overall and recurrence-free survival did not correlate with intra-and peritumoral CD3 and CD8 T-cell infiltration. In addition, no significant association regarding prognosis was found for tumor-associated neutrophils, tumor-associated macrophages or PD-L1 positivity in MCCs. Interestingly, the presence of tertiary lymphoid structures (TLS) in the tumor microenvironment significantly correlated with recurrence-free survival (P=0.025). In addition, TLS were significantly associated with a higher CD8/CD4 ratio in the tumor periphery (P=0.032), but not in the center of the tumor (P > 0.999). These results demonstrate for the first time that TLS, easily assessed in paraffin-embedded tissue in the tumor periphery of MCCs, may be a valuable prognostic factor indicating prolonged recurrence free survival.
Nectin‐2 is an adhesion molecule that has been reported to play a role in tumor growth, metastasis and tumor angiogenesis. Herein, we investigated Nectin‐2 in ovarian cancer patients and in cell culture. Tumor as well as peritoneal biopsies of 60 ovarian cancer patients and 22 controls were dual stained for Nectin‐2 and CD31 using immunohistochemistry. Gene expression of Nectin‐2 was quantified by real‐time PCR and differences analyzed in relation to various tumor characteristics. In the serum of patients, vascular endothelial growth factor (VEGF) was quantified by ELISA. Effect of VEGF on Nectin‐2 expression as well as permeability was investigated in HUVEC. In tumor biopsies, Nectin‐2 protein was mainly localized in tumor cells, whereas in peritoneal biopsies, clear colocalization was found in the vasculature. T3 patients had a significantly higher percentage of positive lymph nodes and this correlated with survival. Nectin‐2 was significantly upregulated in tumor biopsies in patients with lymph node metastasis and with residual tumor >1 cm after surgery. Nectin‐2 expression was significantly suppressed in the peritoneal endothelium of patients associated with significantly increased VEGF serum levels. In cell culture, VEGF stimulation led to a significant downregulation of Nectin‐2 which was reversed by VEGF‐inhibition. In addition, Nectin‐2 knockdown in endothelial cells was associated with significantly increased endothelial permeability. Nectin‐2 expression in ovarian cancer may support tumor cell adhesion, leading to growth and lymph node metastasis. In addition, VEGF‐induced Nectin‐2 suppression in peritoneal endothelium may support an increase in vascular permeability leading to ascites production.
While interplay between BRCA1 and AURKA-RHAMM-TPX2-TUBG1 regulates mammary epithelial polarization, common genetic variation in HMMR (gene product RHAMM) may be associated with risk of breast cancer in BRCA1 mutation carriers. Following on these observations, we further assessed the link between the AURKA-HMMR-TPX2-TUBG1 functional module and risk of breast cancer in BRCA1 or BRCA2 mutation carriers. Forty-one single nucleotide polymorphisms (SNPs) were genotyped in 15,252 BRCA1 and 8,211 BRCA2 mutation carriers and subsequently analyzed using a retrospective likelihood approach. The association of HMMR rs299290 with breast cancer risk in BRCA1 mutation carriers was confirmed: per-allele hazard ratio (HR) = 1.10, 95% confidence interval (CI) 1.04 - 1.15, p = 1.9 x 10\(^{-4}\) (false discovery rate (FDR)-adjusted p = 0.043). Variation in CSTF1, located next to AURKA, was also found to be associated with breast cancer risk in BRCA2 mutation carriers: rs2426618 per-allele HR = 1.10, 95% CI 1.03 - 1.16, p = 0.005 (FDR-adjusted p = 0.045). Assessment of pairwise interactions provided suggestions (FDR-adjusted p\(_{interaction}\) values > 0.05) for deviations from the multiplicative model for rs299290 and CSTF1 rs6064391, and rs299290 and TUBG1 rs11649877 in both BRCA1 and BRCA2 mutation carriers. Following these suggestions, the expression of HMMR and AURKA or TUBG1 in sporadic breast tumors was found to potentially interact, influencing patients' survival. Together, the results of this study support the hypothesis of a causative link between altered function of AURKA-HMMR-TPX2-TUBG1 and breast carcinogenesis in BRCA1/2 mutation carriers.
Background
The decision making process for axillary dissection has changed in recent years for patients with early breast cancer and positive sentinel lymph nodes (LN). The question now arises, what is the optimal surgical treatment for patients with positive axillary LN (pN+). This article tries to answer the following questions:

Is there a survival benefit for breast cancer patients with 3 or more positive LN (pN3+) and with more than 10 removed LN?
(2)
Is there a survival benefit for high risk breast cancer patients (triple negative or Her2 + breast cancer) and with 3 or more positive LN (pN3+) with more than 10 removed LN?
(3)
In pN + patients is the prognostic value of the lymph node ratio (LNR) of pN+/pN removed impaired if 10 or less LN are removed?
Methods
A retrospective database analysis of the multi center cohort database BRENDA (breast cancer under evidence based guidelines) with data from 9625 patients from 17 breast centers was carried out. Guideline adherence was defined by the 2008 German National consensus guidelines.
Results
2992 out of 9625 patients had histological confirmed positive lymph nodes. The most important factors for survival were intrinsic sub types, tumor size and guideline adherent chemo- and hormonal treatment (and age at diagnosis for overall survival (OAS)). Uni-and multivariable analyses for recurrence free survival (RFS) and OAS showed no significant survival benefit when removing more than 10 lymph nodes even for high-risk patients. The mean and median of LNR were significantly higher in the pN+ patients with ≤10 excised LN compared to patients with > 10 excised LN. LNR was in both, uni-and multivariable, analysis a highly significant prognostic factor for RFS and OAS in both subgroups of pN + patients with less respective more than 10 excised LN. Multivariable COX regression analysis was adjusted by age, tumor size, intrinsic sub types and guideline adherent adjuvant systemic therapy.
Conclusion
The removal of more than 10 LN did not result in a significant survival benefit even in high risk pN + breast cancer patients.
Acute myeloid leukemia (AML) is characterized by recurrent genetic events. The BCL6 corepressor (BCOR) and its homolog, the BCL6 corepressor-like 1 (BCORL1), have been reported to be rare but recurrent mutations in AML. Previously, smaller studies have reported conflicting results regarding impacts on outcomes. Here, we retrospectively analyzed a large cohort of 1529 patients with newly diagnosed and intensively treated AML. BCOR and BCORL1 mutations were found in 71 (4.6%) and 53 patients (3.5%), respectively. Frequently co-mutated genes were DNTM3A, TET2 and RUNX1. Mutated BCORL1 and loss-of-function mutations of BCOR were significantly more common in the ELN2017 intermediate-risk group. Patients harboring loss-of-function mutations of BCOR had a significantly reduced median event-free survival (HR = 1.464 (95%-Confidence Interval (CI): 1.005–2.134), p = 0.047), relapse-free survival (HR = 1.904 (95%-CI: 1.163–3.117), p = 0.01), and trend for reduced overall survival (HR = 1.495 (95%-CI: 0.990–2.258), p = 0.056) in multivariable analysis. Our study establishes a novel role for loss-of-function mutations of BCOR regarding risk stratification in AML, which may influence treatment allocation.
Ziel der vorliegenden Arbeit war es, den Einfluß operativ versorgter abdomineller Aneurysmarupturen auf die Lebensqualität zu evaluieren. Als Kontrollgruppe dienten zum einen Patienten mit elektiv operierten Aortenaneurysmen, zum anderen die gesunde Normalbevölkerung. Des weiteren sollten Faktoren, die das postoperative Überleben nach Aneurysmaruptur beeinflussen können untersucht werden. 35 von 52 Patienten (67,3%) wurden am Tag der Primärdiagnose im Rupturstadium operiert. Bei 17 war das Aneurysma bereits bekannt. Zwischen Primärdiagnose und Ruptur vergingen durchschnittlich 10,5 Monate. Es lagen 42 infrarenale (80,8%) und 9 interrenale (17,3%) Aneurysmen vor. Bei 45 Patienten (86,5%) fand sich eine retroperitoneale Blutung, bei einem eine aortovavale und bei einem eine aortoenterische Fistel. Zwei Aneurysmen (3,8%) rupturierten in die freie Bauchhöhle. Fünf Patienten verstarben vor Erfassung der Rupturform. 46 (88,5%) von 52 Patienten konnten operiert werden. Hierbei wurde 17-mal eine Rohrprothese (37%), 16-mal eine aortobiiliakale (34,8%) und 13-mal (28,3%) eine aortobifemorale Prothese verwendet. Die 30-Tage-Letalität betrug 43,5%. Das mittlere Überleben lag im Gesamtbeobachtungszeitraum (maximal 98,5 Monate) bei 27,3 Monaten, das mediane Überleben bei 1,27 Monaten, d.h. die Hälfte der Patienten überlebte weniger als 40 Tage. Die durchschnittliche Operationsdauer lag bei 163 Minuten. Weder die Art des operativen Eingriffs, die Bekanntheit der Primärdiagnose, Revisionseingriffe noch die Lokalisation des Aneurysmas hatten Einfluß auf das Überleben. Dies traf lediglich für die Art des operativen Vorgehens zu. Bei der Verwendung einer aortobiiliakalen Prothese wurden die besten Ergebnisse erzielt. Ein signifikanter Einfluß auf die 30-Tage-Letalität konnte auch für den Ausbildungsstand des Operateurs ermittelt werden.Von allen erhobenen Risikofaktoren hatte nur der Hypertonus signifikanten Einfluß auf das Überleben. Die 30-Tage-Überlebenskurve wird auch durch das Alter bei Operation beeinflusst. Ein Alter über 75 Jahren wirkte sich negativ aus. Hinsichtlich aufgetretener postoperativer Komplikationen konnte in unserem Patientkollektiv kein Einfluß auf das Überleben ermittelt werden. Lediglich das akute Nierenversagen verfehlte knapp das Signifikanzniveau. Auch Durchmesser und Längenausdehnung hatten keine Auswirkung. Insgesamt konnten Daten von 37 Patienten bzgl. der Lebensqualität erhoben werden. Diese wurden in zwei Gruppen unterteilt. Von den 52 Patienten mit Aneurysmaruptur lebten zum Zeitpunkt der Untersuchung noch 11. 8 waren bereit, an der Studie teilzunehmen. Bei 29 Patienten war ein elektiver Eingriff erfolgt. Das Durchschnittsalter bei Operation lag in der Patientengruppe mit Ruptur bei 73,2 (55-86) Jahren. Die Patienten mit elektivem Eingriff waren durchschnittlich 72,1 (50-81) Jahre. In beiden Gruppen waren zwei Frauen. Die Lebensqualität wurde anhand der allgemein gesundheitsbezogenen Fragebögen SF-36, NHP, WHOQOL und HADS-d von den Patienten selbst eingeschätzt. Patienten mit Aneurysmaruptur waren v.a. in der Mobilität, Energie und bei der Bewältigung von Alltagsaufgaben stärker eingeschränkt als Patienten mit elektivem Eingriff. Hinsichtlich „Angst“ und „Depressivität“ ergaben sich keine Unterschiede.Die Elektivgruppe unterschied sich nicht von der gesunden Normalbevölkerung. Patienten mit Aneurysmaruptur schnitten in den Bereichen „Mobilität“, „Alltag“ und „Energie“ schlechter ab als die gesunde Normalbevölkerung. Außerdem ergaben sich im HADS-d schlechtere Werte in der Subskala „Angst“.
Leukemia inhibitory factor (LIF) and Ciliary Neurotrophic factor (CNTF) are members of the interleukin-6 family of cytokines, defined by use of the gp130 molecule as an obligate receptor. In the murine experimental autoimmune encephalomyelitis (EAE) model, antagonism of LIF and genetic deletion of CNTF worsen disease. The potential mechanism of action of these cytokines in EAE is complex, as gp130 is expressed by all neural cells, and could involve immuno-modulation, reduction of oligodendrocyte injury, neuronal protection, or a combination of these actions. In this study we aim to investigate whether the beneficial effects of CNTF/LIF signalling in EAE are associated with axonal protection; and whether this requires signalling through oligodendrocytes. We induced MOG\(_{35-55}\) EAE in CNTF, LIF and double knockout mice. On a CNTF null background, LIF knockout was associated with increased EAE severity (EAE grade 2.1\(\pm\)0.14 vs 2.6\(\pm\)0.19; P<0.05). These mice also showed increased axonal damage relative to LIF heterozygous mice, as indicated by decreased optic nerve parallel diffusivity on MRI (1540\(\pm\)207 \(\mu\)m\(^2\)-/s vs 1310\(\pm\)175 \(\mu\)m\(^2\)-/s; P<0.05), and optic nerve (-12.5%) and spinal cord (-16%) axon densities; and increased serum neurofilament-H levels (2.5 fold increase). No differences in inflammatory cell numbers or peripheral auto-immune T-cell priming were evident. Oligodendrocyte-targeted gp130 knockout mice showed that disruption of CNTF/LIF signalling in these cells has no effect on acute EAE severity. These studies demonstrate that endogenous CNTF and LIF act centrally to protect axons from acute inflammatory destruction via an oligodendrocyte-independent mechanism.
Maligne Tumore der großen und kleinen Speicheldrüsen stellen nur einen geringen Anteil aller Tumore im Mund-, Kiefer- und Gesichtsbereich, weshalb Studien häufig geringe Fallzahlen und Beobachtungszeiträume aufweisen. Ziel dieser Arbeit war daher die Evaluation der Langzeitergebnisse des Würzburger Patientenkollektivs mit Speicheldrüsenkarzinomen. In der vorliegenden Studie konnten die Krankenakten von insgesamt 101 Patienten mit Speicheldrüsenmalignomen, welche über einen Zeitraum von 25 Jahren (1981 - 2006) an der Klinik und Poliklinik für Mund-, Kiefer- uns Gesichtschirurgie der Universität Würzburg behandelt wurden, retrospektiv evaluiert und mit den Daten anderer Autoren verglichen werden. Das Verhältnis von männlichen zu weiblichen Patienten betrug 48 zu 53, das durchschnittliche Alter lag bei 55,5 ± 14 Jahren mit einem Minimum von 15,4 und einem Maximum von 83,8 Jahren. Die Verteilungen der Tumorarten auf die großen und kleinen Speicheldrüsen sowie auf das Gesamtkollektiv entsprechen weitestgehend den Angaben in der Literatur: 46,5% der Patienten litten an einem adenoidzystisches Karzinom, 25,7% an einem low- oder high-grade-Mukoepidermoidkarzinom, 11,9% an einem Adenokarzinom, 5,9% an einem Plattenepithelkarzinom, 3,0% an einem Azinuszellkarzinom, insgesamt 6,9% wiesen weitere Subtypen mit geringeren Fallzahlen auf und wurden deskriptiv evaluiert. Innerhalb des Kollektivs der hochmalignen Speicheldrüsentumore konnten 74,4% der Patienten R0-reseziert werden, 28,2% wurden bestrahlt. Bei 78,3% der Patienten mit einem niedrigmalignen Speicheldrüsentumor konnte eine R0-Resektion erreicht werden. Eine Bestrahlung wurde bei dieser Gruppe in 4,3% der Fälle durchgeführt. Die für die statistische Betrachtung der Überlebenskurven herangezogenen Nachsorgeintervalle lagen zwischen 0,1 und 22,8 Jahren. Das tumorfreie Überleben aller Patienten nach 5, 10, 15 und 20 Jahren betrug 69,9%, 51,4%, 34,2% und 25,6%, der Anteil an Patienten ohne Lokalrezidiv zu den jeweiligen Zeitpunkten 69,9%, 51,4%, 34,2% und 25,6%, wobei Patienten mit niedrigmalignen Tumoren oder niedrigem UICC-Stadium signifikant länger überlebten (p<0,001). Am längsten überlebten Patienten mit einem low-grade-Mukoepidermoidkarzinom, gefolgt von Patienten mit Azinuszellkarzinom, adenoidzystischem Karzinom, Plattenepithelkarzinom, high-grade-Mukoepidermoidkarzinum, Adenokarzinom und Karzinom im pleomorphen Adenom. Die Fallzahl in einigen Subgruppen, wie dem Azinuszellkarzinom und dem undifferenzierten Karzinom sind allerdings gering, so dass die statistische Aussagekraft insbesondere über den Einfluss der Strahlentherapie auf das Überleben gering erscheint und weitere Daten besonders mit modernen Strahlentherapieprotokollen gesammelt werden sollten. Abgesehen davon ist die alleinige Betrachtung der 5-Jahres-Überlebensraten äußerst fraglich, da bei Speicheldrüsenmalignomen auch nach Jahrzehnten noch Rezidive auftreten können. Diese Tatsache macht eine lange Tumornachsorge erforderlich, damit den besonderen tumorbiologischen Eigenschaften Rechnung getragen werden kann. Übereinstimmend mit der internationalen Literatur hatten Tumorhistologie, Tumorstadium und Resektionsstatus mit p<0,001 signifikanten Einfluss auf die Prognose. Zur besseren Erfassung der Einflüsse adjuvanter Therapiekonzepte und zur Erhöhung der Fallzahlen dieser seltenen Tumorentität sollten weitere prospektive, randomisierte Multicenterstudien durchgeführt werden.
The vast majority of chronic myeloid leukemia patients express a BCR-ABL1 fusion gene mRNA encoding a 210 kDa tyrosine kinase which promotes leukemic transformation. A possible differential impact of the corresponding BCR-ABL1 transcript variants e13a2 ("b2a2") and e14a2 ("b3a2") on disease phenotype and outcome is still a subject of debate. A total of 1105 newly diagnosed imatinib-treated patients were analyzed according to transcript type at diagnosis (e13a2, n=451; e14a2, n=496; e13a2+e14a2, n=158). No differences regarding age, sex, or Euro risk score were observed. A significant difference was found between e13a2 and e14a2 when comparing white blood cells (88 vs. 65 x 10(9)/L, respectively; P<0.001) and platelets (296 vs. 430 x 109/L, respectively; P<0.001) at diagnosis, indicating a distinct disease phenotype. No significant difference was observed regarding other hematologic features, including spleen size and hematologic adverse events, during imatinib-based therapies. Cumulative molecular response was inferior in e13a2 patients (P=0.002 for major molecular response; P<0.001 for MR4). No difference was observed with regard to cytogenetic response and overall survival. In conclusion, e13a2 and e14a2 chronic myeloid leukemia seem to represent distinct biological entities. However, clinical outcome under imatinib treatment was comparable and no risk prediction can be made according to e13a2 versus e14a2 BCR-ABL1 transcript type at diagnosis. Emotionales Befinden, Krankheitsverarbeitung und Überlebenszeit bei Bronchialkarzinompatienten

Die vorliegende hypothesenprüfende Arbeit hat als erweiterte Replikationsstudie zum Ziel, den Zusammenhang zwischen emotionalem Befinden und Krankheitsverarbeitung bei Bronchialkarzinomkranken zu untersuchen und die Befunde bezüglich Krankheitsverarbeitung und Überlebenszeit aus der Vorstudie von Faller et al. (1999) beziehungsweise Faller und Bülzebruck (2002) zu überprüfen. 59 an einem Bronchialkarzinom erkrankte Patienten wurden vor Beginn der Primärbehandlung zu ihrem emotionalen Befinden und der Art ihrer Krankheitsbewältigung befragt. Neben der Selbsteinschätzung fand eine Fremdeinschätzung durch die Interviewer statt. Die Überlebensdaten wurden nach 3 - 5 Jahren erhoben. In der vorliegenden Studie konnte ein Zusammenhang zwischen emotionaler Belastung und depressiver Krankheitsverarbeitung nachgewiesen werden. Die Untersuchung zeigte zudem, dass unter Einbeziehung biomedizinischer prognostischer Faktoren eine selbsteingeschätzte depressive Krankheitsverarbeitung mit einer kürzeren Überlebenszeit einhergeht(hazard ratio 1.75, 95% confidence interval 1.04-2.93, p = 0.034). Der körperliche Leistungszustand (Karnofsky-Index) stellte ebenfalls einen unabhängigen Prädiktor für die Überlebenszeit dar. Diese Ergebnisse stimmen mit denjenigen der Vorstudie von Faller und Bülzebruck (2002) überein. Einschränkungen der Studie bestehen aufgrund der relativ kleinen Stichprobe sowie deren hoher Selektivität.
The activation of immune cells by targeting checkpoint inhibitors showed promising results with increased patient survival in distinct primary cancers. Since only limited data exist for human brain metastases, we aimed at characterizing tumor infiltrating lymphocytes (TILs) and expression of immune checkpoints in the respective tumors. Two brain metastases cohorts, a mixed entity cohort (n = 252) and a breast carcinoma validation cohort (n = 96) were analyzed for CD3+, CD8+, FOXP3+, PD-1+ lymphocytes and PD-L1+ tumor cells by immunohistochemistry. Analyses for association with clinico-epidemiological and neuroradiological parameters such as patient survival or tumor size were performed. TILs infiltrated brain metastases in three different patterns (stromal, peritumoral, diffuse). While carcinomas often show a strong stromal infiltration, TILs in melanomas often diffusely infiltrate the tumors. Highest levels of CD3+ and CD8+ lymphocytes were seen in renal cell carcinomas (RCC) and strongest PD-1 levels on RCCs and melanomas. High amounts of TILs, high ratios of PD-1+/CD8+ cells and high levels of PD-L1 were negatively correlated with brain metastases size, indicating that in smaller brain metastases CD8+ immune response might get blocked. PD-L1 expression strongly correlated with TILs and FOXP3 expression. No significant association of patient survival with TILs was observed, while high levels of PD-L1 showed a strong trend towards better survival in melanoma brain metastases (Log-Rank p = 0.0537). In summary, melanomas and RCCs seem to be the most immunogenic entities. Differences in immunotherapeutic response between tumor entities regarding brain metastases might be attributable to this finding and need further investigation in larger patient cohorts.
Die Zielsetzung dieser Studie ist, die operativen Daten, die Folgen, die Komplikationen, die Langzeit-Nierenfunktion und das Überleben der pelvinen Exenteration retrospektiv zu analysieren. Es wurde eine Gegenüberstellung der Behandlungsergebnisse von inkontinenten mit kontinenten Harnableitungen durchgeführt, um das aufwendigere Verfahren der kontinenten Form kritisch betrachten zu können.
Im Zeitraum von 1992 bis 2013 wurden 64 Exenterationen in der Klinik und Poliklinik für Urologie und Kinderurologie der Universität Würzburg aufgrund nicht-urothelialer Malignome durchgeführt. Das mediane Alter des gesamten Patientenkollektivs lag bei 65 Jahren. Hierunter befanden sich 50 vordere und 14 komplette Exenterationen. Eine Harnableitung durch Anlage der inkontinenten Form erfolgte in 39 und durch Anlage der kontinenten Form in 25 Fällen. Das breite Spektrum der Malignomentitäten des gesamten Kollektivs umfasste nicht-urotheliale Malignome der Zervix, des Uterus, der Vulva, der Prostata, der Harnblase und des Darms. Bei 24 Patienten (37,5%) gelang keine R0-Resektion, und bei 18 Patienten (28,1%) konnte ein Lymphknotenbefall nachgewiesen werden. Die Frühkomplikationsrate betrug 58,8%. In einem Zeitraum von 365 Tagen nach Exenteration lag der mediane Clavien-Wert bei 2 Punkten. Die perioperative Sterblichkeit lag bei 0% und die Tumorprogressionsrate bei 48,4%.
Die Analyse des Überlebens ergab eine mediane Gesamtüberlebenszeit von 30 Monaten und eine 5-Jahres-Gesamtüberlebensrate von 42,7% über das gesamte Kollektiv hinweg. Die tumorspezifische 5-JahresÜberlebensrate betrug 55,6%, und eine R0-Resektion erwies sich als hochsignifikante Einflussgröße bezüglich der tumorspezifischen Überlebenszeit. Ein signifikanter Einfluss des Lymphknotenbefalls konnte nicht nachgewiesen werden. Der Einfluss der Komorbidität erwies sich als noch geringer.
Die beiden Kollektive der Harnableitungsformen unterschieden sich signifikant in Komorbiditätsgrad, OP-Dauer, Hospitalisierungszeit und bezüglich der Harnableitungskomplikationen. Die Unterschiede der Komorbidität und der OP-Dauer waren sogar hochsignifikant. Dabei wiesen die Patienten mit Anlage eines kontinenten Verfahrens eine niedrigere Komorbidität, eine längere OP-Dauer, eine längere Hospitalisierungszeit und prozentual mehr Komplikationen bezüglich der Harnableitung auf. Weitere wichtige Parameter, in denen sich die Kollektive geringfügig unterschieden, waren das Alter und die ASA-Klassifikation. Das Kollektiv mit Anlage einer kontinenten Form war jünger und zeigte einen kleineren Wert bezüglich der präoperativen Risikoeinschätzung. Diese Parameter unterschieden sich jedoch nicht signifikant voneinander. Die inkontinente Harnableitung zeigte einen etwas höheren Anteil an weiter fortgeschrittenen Tumorstadien, und nur in diesem Kollektiv lagen präoperativ Metastasen vor. Bei den Früh- und Spätkomplikationen konnte kein nennenswerter Unterschied zwischen den beiden Kollektiven nachgewiesen werden. Nur um wenige Prozentpunkte war die Frühkomplikationsrate der inkontinenten Form (61,3%) höher als die der kontinenten (55,0%). Um den Schweregrad der Komplikationen miteinzubeziehen, wurde der mediane Clavien-Wert aller Komplikationen innerhalb von 365 Tagen erfasst. Er betrug in beiden Kollektiven 2 Punkte.
Bei der Analyse des Überlebens zeigte sich, dass das Kollektiv mit Anlage einer kontinenten Form eine knapp über dem Signifikanzlevel höhere Überlebenswahrscheinlichkeit sowohl bezüglich der gesamten als auch der progressionsfreien Überlebenszeit im Vergleich zu den inkontinenten Verfahren aufwies. Allerdings waren die Unterschiede nicht signifikant und beide Gruppen heterogen bezüglich des Alters, der Komorbidität, den Tumorstadien und den Malignomentitäten.
Die vorliegende Studie kommt zu dem Ergebnis, dass R0-Resektionen bei exenterativen Eingriffen eine essentielle Voraussetzung für das langfristige tumorspezifische Überleben darstellen. In beiden Kollektiven der verschiedenen Harnableitungen zeigte sich kein bedeutsamer Unterschied bezüglich der Komplikationen. Die geringere OP-Dauer und die geringere Anzahl an Komplikationen mit der Harnableitung sprechen für das inkontinente Verfahren. Die Überlebensraten zeigten bessere Ergebnisse für die kontinente Form, jedoch waren die Unterschiede nicht signifikant. Dennoch ist eine Bevorzugung des kontinenten Verfahrens, wenn es technisch möglich und onkologisch vertretbar ist, nach intensiver Beratung und unter Berücksichtigung des Zustandes sowie der Wünsche des Patienten durchaus gerechtfertigt. Aufgrund des nichtrandomisierten retrospektiven Charakters dieser Studie, die 2 heterogene Kollektive vergleicht, sollten idealerweise prospektiv angelegte Studien mit größerer Patientenanzahl in der Zukunft klären, ob die hier gefundenen Ergebnisse generelle Gültigkeit haben.
Background The precise definition of the post-operative resection status in high-grade gliomas (HGG) is crucial for further management. We aimed to assess the feasibility of assessment of the resection status with early post-operative positron emission tomography (PET) using [\(^{18}\)F]O-(2-[\(^{18}\)F]-fluoroethyl)-L-tyrosine ([\(^{18}\)F]FET). Methods 25 patients with the suspicion of primary HGG were enrolled. All patients underwent preoperative [\(^{18}\)F]FET-PET and magnetic resonance imaging (MRI). Intra-operatively, resection status was assessed using 5-aminolevulinic acid (5-ALA). Imaging was repeated within 72h after neurosurgery. Post-operative [\(^{18}\)F]FET-PET was compared with MRI, intra-operative assessment and clinical follow-up. Results [\(^{18}\)F]FET-PET, MRI and intra-operative assessment consistently revealed complete resection in 12/25 (48%) patients and incomplete resection in 6/25 cases (24%). In 7 patients, PET revealed discordant findings. One patient was re-resected. 3/7 experienced tumor recurrence, 3/7 died shortly after brain surgery. Conclusion Early assessment of the resection status in HGG with [\(^{18}\)F]FET-PET seems to be feasible.
Background: RET (rearranged during transfection) variants are the most prevalent oncogenic events in medullary thyroid cancer (MTC). In advanced disease, multi-tyrosine kinase inhibitors (MKIs) cabozantinib and vandetanib are the approved standard treatment irrespective of RET status. The actual outcome of patients with RET-positive MTC treated with MKIs is ill described. Methods: We here retrospectively determined the RET oncogene variant status with a targeted DNA Custom Panel in a prospectively collected cohort of 48 patients with advanced MTC treated with vandetanib and/or cabozantinib at four German referral centers. Progression-free survival (PFS) and overall survival (OS) probabilities were estimated using the Kaplan-Meier method. Results: In total, 44/48 (92%) patients had germline or somatic RET variants. The M918T variant was found in 29/44 (66%) cases. In total, 2/32 (6%) patients with a somatic RET variant had further somatic variants, while in 1/32 (3%) patient with a germline RET variant, additional variants were found. Only 1/48 (2%) patient had a pathogenic HRAS variant, and no variants were found in 3 cases. In first-line treatment, the median OS was 53 (95% CI (95% confidence interval), 32–NR (not reached); n = 36), and the median PFS was 21 months (12–39; n = 33) in RET-positive MTC patients. In second-line treatment, the median OS was 18 (13–79; n = 22), and the median PFS was 3.5 months (2–14; n = 22) in RET-positive cases. Conclusions: RET variants were highly prevalent in patients with advanced MTC. The treatment results in RET-positive cases were similar to those reported in unselected cohorts.
(1) Background: Evaluation of impact of adjuvant radiation therapy (RT) in patients with oral squamous cell carcinoma of the oral cavity/oropharynx (OSCC) of up to 4 cm (pT1/pT2) and solitary ipsilateral lymph node metastasis (pN1). A non-irradiated group with clinical follow-up was chosen for control, and survival and quality of life (QL) were compared; (2) Methods: This prospective multicentric comprehensive cohort study included patients with resected OSCC (pT1/pT2, pN1, and cM0) who were allocated into adjuvant radiation therapy (RT) or observation. The primary endpoint was overall survival. Secondary endpoints were progression-free survival and QL after surgery; (3) Results: Out of 27 centers, 209 patients were enrolled with a median follow-up of 3.4 years. An amount of 137 patients were in the observation arm, and 72 received adjuvant irradiation. Overall survival did not differ between groups (hazard ratio (HR) 0.98 [0.55–1.73], p = 0.94). There were fewer neck metastases (HR 0.34 [0.15–0.77]; p = 0.01), as well as fewer local recurrences (HR 0.41 [0.19–0.89]; p = 0.02) under adjuvant RT. For QL, irradiated patients showed higher values for the symptom scale pain after 0.5, two, and three years (all p < 0.05). After six months and three years, irradiated patients reported higher symptom burdens (impaired swallowing, speech, as well as teeth-related problems (all p < 0.05)). Patients in the RT group had significantly more problems with mouth opening after six months, one, and two years (p < 0.05); (4) Conclusions: Adjuvant RT in patients with early SCC of the oral cavity and oropharynx does not seem to influence overall survival, but it positively affects progression-free survival. However, irradiated patients report a significantly decreased QL up to three years after therapy compared to the observation group.
Background
Multimodal treatment strategies – perioperative chemotherapy (CTx) and radical surgery – are currently accepted as treatment standard for locally advanced gastric cancer. However, the role of adjuvant postoperative CTx (postCTx) in addition to neoadjuvant preoperative CTx (preCTx) in this setting remains controversial.
Methods
Between 4/2006 and 12/2013, 116 patients with locally advanced gastric cancer were treated with preCTx. 72 patients (62 %), in whom complete tumor resection (R0, subtotal/total gastrectomy with D2-lymphadenectomy) was achieved, were divided into two groups, one of which receiving adjuvant therapy (n = 52) and one without (n = 20). These groups were analyzed with regard to survival and exclusion criteria for adjuvant therapy.
Results
Postoperative complications, as well as their severity grade, did not correlate with fewer postCTx cycles administered (p = n.s.). Long-term survival was shorter in patients receiving postCTx in comparison to patients without postCTx, but did not show statistical significance. In per protocol analysis by excluding two patients with perioperative death, a shorter 3-year survival rate was observed in patients receiving postCTx compared to patients without postCTx (3-year survival: 71.2 % postCTx group vs. 90.0 % non-postCTx group; p = 0.038).
Conclusion
These results appear contradicting to the anticipated outcome. While speculative, they question the value of post-CTx. Prospectively randomized studies are needed to elucidate the role of postCTx.
CXCR4 is a G-protein-coupled receptor that mediates recruitment of blood cells toward its ligand SDF-1. In cancer, high CXCR4 expression is frequently associated with tumor dissemination andpoor prognosis. We evaluated the novel CXCR4 probe [\(^{68}\)Ga]Pentixafor for invivo mapping of CXCR4 expression density in mice xenografted with human CXCR4-positive MM cell lines and patients with advanced MM by means of positron emission tomography (PET). [\(^{68}\)Ga]Pentixafor PET provided images with excellent specificity and contrast. In 10 of 14 patients with advanced MM [\(^{68}\)Ga]Pentixafor PET/CT scans revealed MM manifestations, whereas only nine of 14 standard [\(^{18}\)F]fluorodeoxyglucose PET/CT scans were rated visually positive. Assessment of blood counts and standard CD34\(^{+}\) flow cytometry did not reveal significant blood count changes associated with tracer application. Based on these highly encouraging data on clinical PET imaging of CXCR4 expression in a cohort of MM patients, we conclude that [\(^{68}\)Ga]Pentixafor PET opens a broad field for clinical investigations on CXCR4 expression and for CXCR4-directed therapeutic approaches in MM and other diseases.
Background
This article summarizes the 2012 European Renal Association—European Dialysis and Transplant Association Registry Annual Report (available at www.era-edta-reg.org) with a specific focus on older patients (defined as ≥65 years).
Methods
Data provided by 45 national or regional renal registries in 30 countries in Europe and bordering the Mediterranean Sea were used. Individual patient level data were received from 31 renal registries, whereas 14 renal registries contributed data in an aggregated form. The incidence, prevalence and survival probabilities of patients with end-stage renal disease (ESRD) receiving renal replacement therapy (RRT) and renal transplantation rates for 2012 are presented.
Results
In 2012, the overall unadjusted incidence rate of patients with ESRD receiving RRT was 109.6 per million population (pmp) (n = 69 035), ranging from 219.9 pmp in Portugal to 24.2 pmp in Montenegro. The proportion of incident patients ≥75 years varied from 15 to 44% between countries. The overall unadjusted prevalence on 31 December 2012 was 716.7 pmp (n = 451 270), ranging from 1670.2 pmp in Portugal to 146.7 pmp in the Ukraine. The proportion of prevalent patients ≥75 years varied from 11 to 32% between countries. The overall renal transplantation rate in 2012 was 28.3 pmp (n = 15 673), with the highest rate seen in the Spanish region of Catalonia. The proportion of patients ≥65 years receiving a transplant ranged from 0 to 35%. Five-year adjusted survival for all RRT patients was 59.7% (95% confidence interval, CI: 59.3–60.0) which fell to 39.3% (95% CI: 38.7–39.9) in patients 65–74 years and 21.3% (95% CI: 20.8–21.9) in patients ≥75 years.
Background:
In head and neck cancer little is known about the kinetics of osteopontin (OPN) expression after tumor resection. In this study we evaluated the time course of OPN plasma levels before and after surgery.
Methods:
Between 2011 and 2013 41 consecutive head and neck cancer patients were enrolled in a prospective study (group A). At different time points plasma samples were collected: T0) before, T1) 1 day, T2) 1 week and T3) 4 weeks after surgery. Osteopontin and TGFβ1 plasma concentrations were measured with a commercial ELISA system. Data were compared to 131 head and neck cancer patients treated with primary (n = 42) or postoperative radiotherapy (n = 89; group B1 and B2).
Results:
A significant OPN increase was seen as early as 1 day after surgery (T0 to T1, p < 0.01). OPN levels decreased to base line 3-4 weeks after surgery. OPN values were correlated with postoperative TGFβ1 expression suggesting a relation to wound healing. Survival analysis showed a significant benefit for patients with lower OPN levels both in the primary and postoperative radiotherapy group (B1: 33 vs 11.5 months, p = 0.017, B2: median not reached vs 33.4, p = 0.031). TGFβ1 was also of prognostic significance in group B1 (33.0 vs 10.7 months, p = 0.003).
Conclusions:
Patients with head and neck cancer showed an increase in osteopontin plasma levels directly after surgery. Four weeks later OPN concentration decreased to pre-surgery levels. This long lasting increase was presumably associated to wound healing. Both pretherapeutic osteopontin and TGFβ1 had prognostic impact.
Objective:
To determine the survival in a population of German patients with Duchenne muscular dystrophy.
Patients and methods:
Information about 94 patients born between 1970 and 1980 was obtained by telephone interviews and questionnaires. In addition to age of death or actual age during the investigation, data concerning clinical course and medical interventions were collected.
Results:
67 patients with molecularly confirmed diagnoses had a median survival of 24.0 years. Patients without molecular confirmation (clinical diagnosis only) had a chance of 67 % to reach that age. Grouping of our patient cohort according to the year of death (before and after 2000), ventilation was recognized as main intervention affecting survival with ventilated reaching a median survival of 27.0 years. For those without ventilation it was 19.0 years.
Conclusion and clinical relevance:
our study provides survival data for a cohort of DMD patients in Germany stratified by year of death. Median survival was 24.0 years in patients confirmed by molecular testing. Ventilated patients had a median survival of 27 years. We consider this piece of information helpful in the medical care of DMD patients.
The cellular microenvironment in follicular lymphoma is of biological and clinical importance. Studies on the clinical significance of non-malignant cell populations have generated conflicting results, which may partly be influenced by poor reproducibility in immunohistochemical marker quantification. In this study, the reproducibility of manual scoring and automated microscopy based on a tissue microarray of 25 follicular lymphomas as compared to flow cytometry is evaluated. The agreement between manual scoring and flow cytometry was moderate for CD3, low for CD4, and moderate to high for CD8, with some laboratories scoring closer to the flow cytometry results. Agreement in manual quantification across the 7 laboratories was low to moderate for CD3, CD4, CD8 and FOXP3 frequencies, moderate for CD21, low for MIB1 and CD68, and high for CD10. Manual scoring of the architectural distribution resulted in moderate agreement for CD3, CD4 and CD8, and low agreement for FOXP3 and CD68. Comparing manual scoring to automated microscopy demonstrated that manual scoring increased the variability in the low and high frequency interval with some laboratories showing a better agreement with automated scores. Manual scoring reliably identified rare architectural patterns of T-cell infiltrates. Automated microscopy analyses for T-cell markers by two different instruments were highly reproducible and provided acceptable agreement with flow cytometry. These validation results provide explanations for the heterogeneous findings on the prognostic value of the microenvironment in follicular lymphoma. We recommend a more objective measurement, such as computer-assisted scoring, in future studies of the prognostic impact of microenvironment in follicular lymphoma patients.
Obligate human pathogenic Neisseria gonorrhoeae are the second most frequent bacterial cause of sexually transmitted diseases. These bacteria invade different mucosal tissues and occasionally disseminate into the bloodstream. Invasion into epithelial cells requires the activation of host cell receptors by the formation of ceramide-rich platforms. Here, we investigated the role of sphingosine in the invasion and intracellular survival of gonococci. Sphingosine exhibited an anti-gonococcal activity in vitro. We used specific sphingosine analogs and click chemistry to visualize sphingosine in infected cells. Sphingosine localized to the membrane of intracellular gonococci. Inhibitor studies and the application of a sphingosine derivative indicated that increased sphingosine levels reduced the intracellular survival of gonococci. We demonstrate here, that sphingosine can target intracellular bacteria and may therefore exert a direct bactericidal effect inside cells.
Recently, several classifiers that combine primary tumor data, like gene expression data, and secondary data sources, such as protein-protein interaction networks, have been proposed for predicting outcome in breast cancer. In these approaches, new composite features are typically constructed by aggregating the expression levels of several genes. The secondary data sources are employed to guide this aggregation. Although many studies claim that these approaches improve classification performance over single genes classifiers, the gain in performance is difficult to assess. This stems mainly from the fact that different breast cancer data sets and validation procedures are employed to assess the performance. Here we address these issues by employing a large cohort of six breast cancer data sets as benchmark set and by performing an unbiased evaluation of the classification accuracies of the different approaches. Contrary to previous claims, we find that composite feature classifiers do not outperform simple single genes classifiers. We investigate the effect of (1) the number of selected features; (2) the specific gene set from which features are selected; (3) the size of the training set and (4) the heterogeneity of the data set on the performance of composite feature and single genes classifiers. Strikingly, we find that randomization of secondary data sources, which destroys all biological information in these sources, does not result in a deterioration in performance of composite feature classifiers. Finally, we show that when a proper correction for gene set size is performed, the stability of single genes sets is similar to the stability of composite feature sets. Based on these results there is currently no reason to prefer prognostic classifiers based on composite features over single genes classifiers for predicting outcome in breast cancer.
Background: Recently published results of quality of life (QoL) studies indicated different outcomes of palliative radiotherapy for brain metastases. This prospective multi-center QoL study of patients with brain metastases was designed to investigate which QoL domains improve or worsen after palliative radiotherapy and which might provide prognostic information.
Methods: From 01/2007-01/2009, n=151 patients with previously untreated brain metastases were recruited at 14 centers in Germany and Austria. Most patients (82 %) received whole-brain radiotherapy. QoL was measured with the EORTC-QLQ-C15-PAL and brain module BN20 before the start of radiotherapy and after 3 months.
Results: At 3 months, 88/142 (62 %) survived. Nine patients were not able to be followed up. 62 patients (70.5 % of 3-month survivors) completed the second set of questionnaires. Three months after the start of radiotherapy QoL deteriorated significantly in the areas of global QoL, physical function, fatigue, nausea, pain, appetite loss, hair loss, drowsiness, motor dysfunction, communication deficit and weakness of legs. Although the use of corticosteroid at 3 months could be reduced compared to pre-treatment (63 % vs. 37 %), the score for headaches remained stable. Initial QoL at the start of treatment was better in those alive than in those deceased at 3 months, significantly for physical function, motor dysfunction and the symptom scales fatigue, pain, appetite loss and weakness of legs. In a multivariate model, lower Karnofsky performance score, higher age and higher pain ratings before radiotherapy were prognostic of 3-month survival.
Conclusions: Moderate deterioration in several QoL domains was predominantly observed three months after start of palliative radiotherapy for brain metastases. Future studies will need to address the individual subjective benefit or burden from such treatment. Baseline QoL scores before palliative radiotherapy for brain metastases may contain prognostic information.
Die extrakorporale Membranoxygenierung ist ein seit Jahrzehnten etabliertes Verfahren, Patienten trotz kardialem und/oder pulmonalem Versagen ein zeitbegrenztes Überleben zu ermöglichen. Obgleich sich an den Grundzügen der Herangehensweise bis heute wenig verändert hat, konnte diese Hochrisikotherapie mithilfe der Entwicklung blutschonenderer Materialien und der Verwendung verbesserter Pumpen und Oxygenatoren zunehmend effizienter gestaltet werden.
Durch eine Überlebensanalyse aller ECMO-Patienten der Datenbank der Klinik für Tho-rax-, Herz- und Thorakale Gefäßchirurgie des Universitätsklinikums Würzburg zwischen 2015 und 2018 (172 Fälle) sollten unabhängige Risikofaktoren für ein negatives Outcome der Therapie identifiziert werden. Insbesondere den Laborparametern während der ersten 72 Stunden am System galt hierbei ein besonderes Augenmerk, aber auch Vorerkrankun-gen, Komplikationen, Substitutionen während der Therapie und weitere Parameter wurden für jeden Patienten individuell ermittelt und tabellarisch festgehalten.
Insgesamt verstarben davon 125 Patienten (72,7%), 47 Patienten (27,3%) überlebten und konnten entlassen werden. Bei 24 der 125 verstorbenen Patienten (14%) konnte zwar ein Weaning vom System erfolgreich durchgeführt werden (>24h), sie verstarben jedoch während des anschließenden stationären Aufenthaltes.
Bei den präinsertionell erhobenen Parametern waren der BMI und der Euroscore II bei verstorbenen Patienten signifikant höher, ebenso wie die Cross-Clamp-Zeit und der SO-FA-Score.
Für die Laborwerte an ECMO ergaben sich für den Serumlaktatspiegel und die Throm-bozytenanzahl der Patienten die signifikantesten Unterschiede. Auch andere Laborparame-ter erwiesen sich in beiden Gruppen als signifikant unterschiedlich: Insbesondere der Quick Wert der überlebenden Patienten war zu Beginn signifikant höher. Auch der Fibri-nogenspiegel der Gruppe der überlebenden Patienten lag ab der 12 Stunden Marke signi-fikant höher.
Verstorbene Patienten erhielten mehr Blutpräparate als Überlebende. Außerdem führte ein dialysepflichtiges Nierenversagen im Laufe der Therapie zu signifikant schlechterem Out-come.
Wider Erwarten waren während der Therapie auftretende Blutungskomplikationen nicht mit schlechterem Outcome assoziiert. Jedoch konnte bei Auftritt von Thromben im Sys-tem, die einen Austausch des Oxygenators/ECMO-Systems nötig machten, sowie Magen-Darm-Ischämien und Kompartmentsyndrom ein klarer Überlebensnachteil erfasst werden.
Abschließend ließ sich mittels multivariater logistischer Regression zeigen, dass der SO-FA-Score, der Serumlaktatspiegel und die Thrombozytenanzahl sowie eine adjuvante I-ABP Implantation und der Bedarf einer Nierenersatztherapie den größten Einfluss auf das Überleben der Patienten hatten.
Es wurde eine Literaturübersicht der Jahre 1966 bis 1996 über Therapieergeb-nisse von Plattenepithelkarzinomen des Oropharynx erstellt. Als Endergebnis wurde die 5-Jahres-Überlebenszeit festgelegt. Die Hauptlokalisationen für Oropharynxtumoren sind mit absteigender Häufigkeit die Tonsillenregion, der Zungengrund, der weiche Gaumen mit Uvula und die Pharynxwand. Die Behandlungsregime umfassten die alleinige Operation, die alleinige externe Strahlentherapie mit oder ohne interstitieller Bestrahlung, die Kombinations-therapie aus Operation und Radiotherapie sowie die kombinierte Behandlung aus Chemotherapie und Bestrahlung mit oder ohne Operation. Über die alleinige Radiotherapie fanden sich die meisten Publikationen, gefolgt von Veröffent-lichungen über die chirurgisch-radiologische Kombinationstherapie. Über die kombinierte Behandlung aus Chemotherapie und Bestrahlung mit oder ohne Ope-ration gab es die geringste Anzahl verwertbarer Veröffentlichungen. Beim Vergleich der verschiedenen Behandlungsarten lieferte die Operation mit nachfolgender Bestrahlung (OP+post-op.RT), die externe Bestrahlung plus interstitieller Radiotherapie (RT+iRT) und die alleinige Operation (all. OP) die besten Gesamtüberlebenszeiten, in denen die Verteilung der Tumorstadien nicht berücksichtigt wurden, für das Oropharynxkarzinom in den 90er Jahren. Für Patientenkollektive mit überwiegend frühen Tumorstadien zeigte die alleinige Operation (all. OP) die deutlichste Verbesserung der Überlebens-zeiten über den Beobachtungszeitraum und die besten Überlebenszeiten in den 90er Jahren, gefolgt von der externen Bestrahlung plus interstitieller Radio-therapie (RT+iRT). Die Kombinationstherapie aus Operation und Bestrahlung (OP&RT) wurde für diese Patientenkollektive nur ausnahmsweise angewendet. Für Patientenkollektive mit überwiegend fortgeschrittenen Tumorstadien liefer-te die Operation mit nachfolgender Bestrahlung (OP+post-op. RT) und die externe Bestrahlung plus interstitieller Radiotherapie (RT+iRT) die besten Überlebenszeiten in den 90er Jahren, wobei es für erstere eine größere Studienanzahl und eine deutlichere Tendenz zu verbesserten Überlebenszeiten über den Beobachtungszeitraum gab. Die kombinierte Behandlung aus Chemo-therapie und Radiotherapie (CT&RT) zeigte in den 90er Jahren deutlich schlechtere Überlebenszeiten. Für Tonsillen- und Zungengrundkarzinomen lieferte die Kombinationstherapie aus Operation und Bestrahlung (OP&RT) die besten Überlebenszeiten in den 90er Jahren sowohl für das Gesamtkollektiv als auch für die überwiegend fortge-schrittenen Tumorstadien, gefolgt von der externen Bestrahlung mit oder ohne interstitieller Radiotherapie (RT+iRT).
Multiple myeloma is a bone marrow plasma cell tumor which is supported by the external growth factors APRIL and IL-6, among others. Recently, we identified eosinophils and megakaryocytes to be functional components of the micro-environmental niches of benign bone marrow plasma cells and to be important local sources of these cytokines. Here, we investigated whether eosinophils and megakaryocytes also support the growth of tumor plasma cells in the MOPC315. BM model for multiple myeloma. As it was shown for benign plasma cells and multiple myeloma cells, IL-6 and APRIL also supported MOPC315. BM cell growth in vitro, IL-5 had no effect. Depletion of eosinophils in vivo by IL-5 blockade led to a reduction of the early myeloma load. Consistent with this, myeloma growth in early stages was retarded in eosinophil-deficient Delta dblGATA-1 mice. Late myeloma stages were unaffected, possibly due to megakaryocytes compensating for the loss of eosinophils, since megakaryocytes were found to be in contact with myeloma cells in vivo and supported myeloma growth in vitro. We conclude that eosinophils and megakaryocytes in the niches for benign bone marrow plasma cells support the growth of malignant plasma cells. Further investigations are required to test whether perturbation of these niches represents a potential strategy for the treatment of multiple myeloma.
