Neuronale Stammzellen wurden kürzlich im unteren Colliculus inferior (CI) identifiziert. Diese Zellen sind von besonderem Interesse, da es keine therapeutischen Optionen für geschädigte neuronale Strukturen gibt. Ziel dieses Forschungsprojekts ist es, das neurogene Potenzial im CI der Ratte von den ersten postnatalen Tagen bis zum Erwachsenenalter zu untersuchen. Der CI von Ratten vom 6. bis zum 48. postnatalen Tag wurde mit Neurosphären-Assays und histologischen Schnitten untersucht. In frei schwimmenden CI-Zellkulturen bildeten sich Neurosphären bei Tieren vom frühen postnatalen Alter bis zum Erwachsenenalter. Die Menge der gebildeten Neurosphären nahm im höheren Alter ab und stieg mit der Anzahl der Zelllinienpassagen. Die Zellen in den Neurosphären und die histologischen Schnitte zeigten eine positive Färbung mit neuronalen Stammzell-Markern (Doublecortin, Sox-2, Musashi-1, Nestin und Atoh1). Dissoziierte Einzelzellen aus den Neurosphären differenzierten und wurden positiv für die neuralen Abstammungsmarker β-III-Tubulin, GFAP und MBP angefärbt. Darüber hinaus wurden neuronalen Stammzell-Marker (Doublecortin, Sox-2, CDK5R1 und Ascl-1) mittels qRT-PCR untersucht. Zusammenfassend lässt sich sagen, dass ein neurogenes Potenzial im CI der Ratte von den frühen postnatalen Tagen bis zum Erwachsenenalter nachgewiesen und bewertet wurde. Die Identifizierung von neuronalen Stammzellen im CI der Ratte und ihre altersspezifischen Merkmale tragen zu einem besseren Verständnis der Entwicklung und der Plastizität der Hörbahn bei und könnten für eine therapeutische Nutzung aktiviert werden.
Adipose tissue-derived stromal cells (ASCs) represent a capable source for cell-based therapeutic approaches. For monitoring a cell-based application in vivo, magnetic resonance imaging (MRI) of cells labeled with iron oxide particles is a common method. It is the aim of the present study to analyze potential DNA damage, cytotoxicity and impairment of functional properties of human (h)ASCs after labeling with citrate-coated very small superparamagnetic iron oxide particles (VSOPs). Cytotoxic as well as genotoxic effects of the labeling procedure were measured in labeled and unlabeled hASCs using the MTT assay, comet assay and chromosomal aberration test. Trilineage differentiation was performed to evaluate an impairment of the differentiation potential due to the particles. Proliferation as well as migration capability were analyzed after the labeling procedure. Furthermore, the labeling of the hASCs was confirmed by Prussian blue staining, transmission electron microscopy (TEM) and high-resolution MRI. Below the concentration of 0.6 mM, which was used for the procedure, no evidence of genotoxic effects was found. At 0.6 mM, 1 mM as well as 1.5 mM, an increase in the number of chromosomal aberrations was determined. Cytotoxic effects were not observed at any concentration. Proliferation, migration capability and differentiation potential were also not affected by the procedure. Labeling with VSOPs is a useful labeling method for hASCs that does not affect their proliferation, migration and differentiation potential. Despite the absence of cytotoxicity, however, indications of genotoxic effects have been demonstrated.
Vorliegende Arbeit beschäftigte sich mit der Verbesserung von Defiziten der elektrischen Stimulation durch Cochlea Implantate (CI) mit alternativen Pulsformen. Dabei wurde mit elektrophysiologischen und psychophysikalischen Methoden untersucht, wie sich die Pulsformen auf die Effektivität der Stimulation auswirken. Es wurden präzisions-triphasische Pulse (pTP) mit anodischer und kathodischer zweiter Phase anhand der Daten von elf Probanden untersucht.
Im Rahmen der objektiven elektrophysiologischen Messung wurde mit den unterschiedlichen Formen des pTP an drei unterschiedlichen Kontaktpositionen auf den CI-Elektrodenträgern stimuliert, und die Stärke der jeweils evozierten neuronalen Antwort aufgezeichnet. Der subjektive psychophysikalische Test diente dazu, die pulsformspezifischen Hörschwellen zu bestimmen und wurde an zwei unterschiedlichen Kontakten auf den CI-Elektrodenträgern durchgeführt.
Dabei erzielten pTP, welche eine symmetrisch-triphasische Pulsform aufwiesen, geringere neuronale Antwortstärken und höhere Hörschwellen als die pTP, die einer biphasischen Pulsform glichen. Diejenigen pTP, die biphasischen Pulsen mit anodischer erster Phase glichen, erzielten dabei die höchsten neuronalen Antwortstärken und die niedrigsten Hörschwellen.
Hintergrund
Die E‑Zigarette erfreut sich in den letzten Jahren zunehmender Beliebtheit. Die Frage nach der Toxizität ist jedoch noch nicht eindeutig geklärt, und es herrscht global Unsicherheit im Umgang mit der E‑Zigarette.
Ziel
Ziel der vorliegenden Arbeit war es, Propylenglykol, ein Hauptbestandteil der Liquide, in Bezug auf mögliche akute Entzündungsreaktionen, zyto- und genotoxische Auswirkungen auf humane Nasenschleimhautzellen zu untersuchen.
Material und Methoden
Die Nasenschleimhautzellen wurden von zehn Probanden im Air-Liquid-Interface kultiviert und anschließend mit unterschiedlichen Konzentrationen des Propylenglykols bedampft. Die Analyse erfolgte mittels Trypanblau-Test, Comet-Assay, Mikrokerntest und IL-6- und IL-8-Sandwich-ELISA.
Ergebnis
Der Trypanblau-Test zeigte keine Reduktion der Vitalität. Im Sandwich-ELISA konnte kein Anstieg der IL-6- und IL-8-Konzentrationen nachgewiesen werden. Im Comet-Assay zeigte das Olive Tail Moment eine Schädigung im Vergleich zur Negativkontrolle in allen untersuchten Konzentrationen. Zudem zeigte sich eine dosisabhängige Schädigung. Im Mikrokerntest konnte ein Unterschied zwischen dem Reinstoff und der Negativkontrolle gefunden werden.
Schlussfolgerung
Es zeigten sich möglicherweise reparable DNS-Schädigungen im Comet-Assay. Im Mikrokerntest konnten diese nur in der Reinstoffkonzentration bestätigt werden. Es sollte ein restriktiver Umgang mit der E‑Zigarette erfolgen, bis insbesondere Langzeitstudien vorliegen. Zudem ist eine eindeutige Deklaration der Inhaltsstoffe der Liquide durch die Hersteller zu fordern, um weitergehende Schädigungspotenziale untersuchen zu können.
Zinc oxide nanoparticles (ZnO-NPs) are widely utilized, for example in manufacturing paints and in the cosmetic industry. In addition, there is raising interest in the application of NPs in stem cell research. However, cytotoxic, genotoxic and pro-inflammatory effects were shown for NPs. The aim of this study was to evaluate the impact of ZnO-NPs on cytokine secretion and differentiation properties of human adipose tissue-derived stromal cells (ASCs). Human ASCs were exposed to the subtoxic concentration of 0.2 mu g/mL ZnO-NPs for 24 h. After four weeks of cultivation, adipogenic and osteogenic differentiation procedures were performed. The multi-differentiation potential was confirmed histologically and using polymerase chain reaction (PCR). In addition, the gene expression of IL-6, IL-8, vascular endothelial growth factor (VEGF) and caspase 3 was analyzed. Over the course of four weeks after ZnO-NPs exposure, no significant differences were detected in the gene expression of IL-6, IL-8, VEGF and caspase 3 compared to non-exposed cells. The differentiation was also not affected by the ZnO-NPs. These findings underline the fact, that functionality of ASCs is likely to be unaffected by ZnO-NPs, despite a long-term disposition of NPs in the cells, supposing that the starting concentration was safely in the non-toxic range. This might provide important information for single-use nanomedical applications of ZnO-NPs.
Entwicklung optimierter Ankopplungsbedingungen von aktiven Mittelohrimplantaten am runden Fenster

Seit Mitte der 90er-Jahre gewinnen aktive Mittelohrimplantate wie die Vibrant sound bridge (VSB) zunehmend an Bedeutung. Sie können zur Rehabilitation von Schallleitungs- und Schallempfindungsstörungen, sowie kombinierter Hörstörungen indiziert sein. Die Vibrant sound bridge stellt eine Alternative für Patienten dar, bei denen aus verschiedenen Gründen eine Unverträglichkeit gegenüber herkömmlichen Hörgeräten besteht. Bei der Implantation des Floating mass transducers (FMT) am runden Fenster zeigte sich in der Klinik neben einer hohen Revisionsrate von bis zu 15,6 % eine hohe Varianz der audiologischen Ergebnisse, was auf eine unkontrollierte und mangelnde Ankopplung des Floating mass transducers an das runde Fenster zurückzuführen ist. Erste Ergebnisse der kontrollierten Ankopplung am runden Fenster mit einem Kamin zeigten vielversprechende Ergebnisse. Die suffiziente Fixation des Kamins an der Rundfensternische stellt eine Herausforderung dar. Es bedarf eines Haftvermittlers, der eine suffiziente Verbindung zwischen dem Titankamin und dem darunter befindlichen Knochen am runden Fenster herstellt. Ziel der Dissertation war die Findung einer geeigneten Haftkomponente zur Fixation des Kamins am runden Fenster. Es wurden der Calciumphosphatzement HydroSet® (Fa. Stryker) und der Glasionomerzement Ketac™ Cem (Fa. ESPE 3M™ Deutschland) untersucht. In Anlehnung an DIN 13990-2 wurden nach Einwirkung physikalischen Stresses in Form von Temperatur und Feuchtigkeit die Scherhaftfestigkeit der Titanzylinder an humanen Felsenbeinknochen gemessen. Es zeigte sich eine schwach signifikant bessere Haftung für das Calciumphosphatzement HydroSet®, bei insgesamt geringer Scherhaftfestigkeit beider Zemente im Vergleich mit anderen zahnheilkundlichen Adhäsiven der Literatur.
Zinkoxid-Nanopartikel (ZnO-NP) finden in vielen Produkten des täglichen Verbrauchs Verwendung. Daten über die toxikologischen Eigenschaften von ZnO-NP werden kontrovers diskutiert. Die menschliche Haut ist in Bezug auf die ZnO-NP Exposition das wichtigste Kontakt-Organ. Intakte Haut stellt eine suffiziente Barriere gegenüber NP dar. Bei defekter Haut ist ein Kontakt zu den proliferierenden Stammzellen möglich, sodass diese als wichtiges toxikologische Ziel für NP darstellen. Das Ziel dieser Dissertation war die Bewertung der genotoxischen und zytotoxischen Effekte an humanen mesenchymalen Stammzellen (hMSC) durch niedrig dosierte ZnO-NP nach 24 stündiger Exposition, repetitiven Expositionen und im Langzeitversuch bis zu 6 Wochen. Zytotoxische Wirkungen von ZnO-NP wurden mit 3-(4,5-Dimethylthiazol-2-yl)-2,5-diphenyltetrazoliumbromid-Test (MTT) gemessen. Darüber hinaus wurde die Genotoxizität durch den Comet-Assay bewertet. Zur Langzeitbeobachtung bis zu 6 Wochen wurde die Transmissionselektronenmikroskopie (TEM) verwendet. Zytotoxizität nach 24-stündiger ZnO-NP-Exposition war ab einer Konzentration von 50 µg/ml nachweisbar. Genotoxizität konnten bereits bei Konzentrationen von 1 und 10 µg/ml ZnO-NP beschrieben werden. Wiederholte Exposition verstärkte die Zyto-, aber nicht die Genotoxizität. Eine intrazelluläre NP-Akkumulation mit Penetration der Zellorganelle wurde bei einer Exposition bis zu 6 Wochen beobachtet. Die Ergebnisse deuten auf zytotoxische und genotoxisches Effekte von ZnO-NP hin. Bereits geringe Dosen von ZnO-NP können bei wiederholter Exposition toxische Wirkungen hervorrufen sowie eine langfristige Zellakkumulation. Diese Daten sollten bei der Verwendung von ZnO-NP an geschädigter Haut berücksichtigt werden.
Magnetic nanoparticles (NPs), such as very small iron oxide NPs (VSOPs) can be used for targeted drug delivery, cancer treatment or tissue engineering. Another important field of application is the labelling of mesenchymal stem cells to allow in vivo tracking and visualization of transplanted cells using magnetic resonance imaging (MRI). For these NPs, however, various toxic effects, as well as functional impairment of the exposed cells, are described. The present study evaluates the influence of VSOPs on the multilineage differentiation ability and cytokine secretion of human adipose tissue derived stromal cells (hASCs) after long-term exposure. Human ASCs were labelled with VSOPs, and the efficacy of the labelling was documented over 4 weeks in vitro cultivation of the labelled cells. Unlabelled hASCs served as negative controls. Four weeks after labelling, adipogenic and osteogenic differentiation was histologically evaluated and quantified by polymerase chain reaction (PCR). Changes in gene expression of IL-6, IL-8, VEGF and caspase 3 were determined over 4 weeks. Four weeks after the labelling procedure, labelled and unlabelled hASCs did not differ in the gene expression of IL-6, IL-8, VEGF and caspase 3. Furthermore, the labelling procedure had no influence on the multidifferentiation ability of hASC. The percentage of labelled cells decreased during in vitro expansion over 4 weeks. Labelling with VSOPs and long-term intracellular disposition probably have no influence on the physiological functions of hASCs. This could be important for the future in vivo use of iron oxide NPs.
Purpose
Flexible endoscopic evaluation of swallowing (FEES\(^{®}\)) is a standard diagnostic tool in dysphagia. The combination of FEES® and narrow band light (narrow band imaging; NBI) provides a more precise and detailed investigation method. So far, this technique could only be performed with the NBI illumination. The new version of the "professional image enhancement technique" (PIET) provides another image enhancing system. This study investigates the eligibility of PIET in the FEES\(^{®}\) procedure.
Methods
Both techniques, NBI and PIET, were compared using a target system. Furthermore, the image enhancement during FEES\(^{®}\) was performed and recorded with the two systems during daily routine.
Results
Performing an image enhancement during FEES\(^{®}\) is possible with both systems PIET and NBI. On the target system, the contrast of the PIET showed a brighter and a more detailed picture. In dysphagia patients, no difference between PIET and NBI was detected.
Conclusion
PIET proved to be non-inferior to NBI during image enhancement FEES\(^{®}\). So far, image enhancement FEES\(^{®}\) was exclusively connected to NBI. With the PIET system, an alternative endoscopy technology is available for certain indications.
Deep neck infections with and without mediastinal involvement: treatment and outcome in 218 patients

Purpose
Infections of the deep neck, although becoming scarcer due to the widespread use of antibiotics, still represent a dangerous and possibly deadly disease, especially when descending into the mediastinum. Due to the different specialities involved in the treatment and the heterogenous presentation of the disease, therapeutic standard is still controversial. This study analyzes treatment and outcome in these patients based on a large retrospective review and proposes a therapeutic algorithm.
Methods
The cases of 218 adult patients treated with deep neck abscesses over a 10-year period at a tertiary university hospital were analyzed retrospectively. Clinical, radiological, microbiological and laboratory findings were compared between patients with and without mediastinal involvement.
Results
Forty-five patients (20.64%) presented with abscess formation descending into the mediastinum. Those patients had significantly (all items p < 0.0001) higher rates of surgical interventions (4.27 vs. 1.11) and tracheotomies (82% vs. 3.4%), higher markers of inflammation (CRP 26.09 vs. 10.41 mg/dl), required more CT-scans (3.58 vs. 0.85), longer hospitalization (39.78 vs 9.79 days) and more frequently needed a change in antibiotic therapy (44.44% vs. 6.40%). Multi-resistant pathogens were found in 6.67% vs. 1.16%. Overall mortality rate was low with 1.83%.
Conclusion
Despite of the high percentage of mediastinal involvement in the present patient collective, the proposed therapeutic algorithm resulted in a low mortality rate. Frequent CT-scans, regular planned surgical revisions with local drainage and lavage, as well as an early tracheotomy seem to be most beneficial regarding the outcome.
Purpose
For further improvements in cochlear implantation, the measurement of the cochlear duct length (CDL) and the determination of the electrode contact position (ECP) are increasingly in the focus of clinical research. Usually, these items were investigated by multislice computed tomography (MSCT). The determination of ECP was only possible by research programs so far. Flat-panel volume computed tomography (fpVCT) and its secondary reconstructions (fpVCT\(_{SECO}\)) allow for high spatial resolution for the visualization of the temporal bone structures. Using a newly developed surgical planning software that enables the evaluation of CDL and the determination of postoperative ECP, this study aimed to investigate the combination of fpVCT and otological planning software to improve the implementation of an anatomically based cochlear implantation.
Methods
Cochlear measurements were performed utilizing surgical planning software in imaging data (MSCT, fpVCT and fpVCT\(_{SECO}\)) of patients with and without implanted electrodes.
Results
Measurement of the CDL by the use of an otological planning software was highly reliable using fpVCT\(_{SECO}\) with a lower variance between the respective measurements compared to MSCT. The determination of the inter-electrode-distance (IED) between the ECP was improved in fpVCT\(_{SECO}\) compared to MSCT.
Conclusion
The combination of fpVCT\(_{SECO}\) and otological planning software permits a simplified and more reliable analysis of the cochlea in the pre- and postoperative setting. The combination of both systems will enable further progress in the development of an anatomically based cochlear implantation.
Purpose
Audiology is an essential service for some patient groups and some interventions. This article sets forth experience-based recommendations for how audiological centers can continue to safely and effectively function during COVID-19.
Methods
The recommendations are the result of panel discussion and are based on the clinical experience of the panelists/authors.
Results
The recommendations cover which patient groups and which interventions should be treated when and whether this can be performed in the clinic or remotely; how to maintain the safety of workplace via optimizing patient flow within the clinic and the sanitation of rooms and equipment; and overcoming communication challenges that COVID-19 intensifies.
Conclusion
For essential audiological services to continue under COVID-19, safety measures must be implemented and maintained, and treatment and communication strategies must be adapted to offset communication difficulties due to personal protective equipment (PPE) and social distancing and to bolster patient confidence. In short, it is vital that staff feel safe, that patients either feel the clinic is safe enough to visit or that remote treatment may be an option, and that clinics and patients have a broad agreement on the urgency of any needed service. We hope that these recommendations help clinics effectively accomplish these goals.
Pilot study on the value of Raman spectroscopy in the entity assignment of salivary gland tumors

Background
The entity assignment of salivary gland tumors (SGT) based on histomorphology can be challenging. Raman spectroscopy has been applied to analyze differences in the molecular composition of tissues. The aim of this study was to evaluate the suitability of RS for entity assignment in SGT.
Methods
Raman data were collected in deparaffinized sections of pleomorphic adenomas (PA) and adenoid cystic carcinomas (ACC). Multivariate data and chemometric analysis were completed using the Unscrambler software.
Results
The Raman spectra detected in ACC samples were mostly assigned to nucleic acids, lipids, and amides. In a principal component-based linear discriminant analysis (LDA) 18 of 20 tumor samples were classified correctly.
Conclusion
In this proof of concept study, we show that a reliable SGT diagnosis based on LDA algorithm appears possible, despite variations in the entity-specific mean spectra. However, a standardized workflow for tissue sample preparation, measurement setup, and chemometric algorithms is essential to get reliable results.
Objective
Growing interest in measuring the cochlear duct length (CDL) has emerged, since it can influence the selection of cochlear implant electrodes. Currently the measurements are performed with ionized radiation imaging. Only a few studies have explored CDL measurements in magnetic resonance imaging (MRI). Therefore, the presented study aims to fill this gap by estimating CDL in MRI and comparing it with multislice computed tomography (CT).
Study Design
Retrospective data analyses of 42 cochleae.
Setting
Tertiary care medical center.
Methods
Diameter (A value) and width (B value) of the cochlea were measured in HOROS software. The CDL and the 2-turn length were determined by the elliptic circular approximation (ECA). In addition, the CDL, the 2-turn length, and the angular length were determined via HOROS software by the multiplanar reconstruction (MPR) method.
Results
CDL values were significantly shorter in MRI by MPR (d = 1.38 mm, P < .001) but not by ECA. Similar 2-turn length measurements were significantly lower in MRI by MPR (d = 1.67 mm) and ECA (d = 1.19 mm, both P < .001). In contrast, angular length was significantly higher in MRI (d = 26.79°, P < .001). When the values were set in relation to the frequencies of the cochlea, no clinically relevant differences were estimated (58 Hz at 28-mm CDL).
Conclusion
In the presented study, CDL was investigated in CT and MRI by using different approaches. Since no clinically relevant differences were found, diagnostics with radiation may be omitted prior to cochlear implantation; thus, a concept of radiation-free cochlear implantation could be established.
This study aimed to evaluate the feasibility and accuracy of electromagnetic navigation at the lateral skull base in combination with flat panel volume computed tomography (fpVCT) datasets. A mastoidectomy and a posterior tympanotomy were performed on 10 samples of fresh frozen temporal bones. For registration, four self-drilling titanium screws were applied as fiducial markers. Multi-slice computed tomography (MSCT; 600 µm), conventional flat panel volume computed tomography (fpVCT; 466 µm), micro-fpVCT (197 µm) and secondary reconstructed fpVCT (100 µM) scans were performed and data were loaded into the navigation system. The resulting fiducial registration error (FRE) was analysed, and control of the navigation accuracy was performed. The registration process was very quick and reliable with the screws as fiducials. Compared to using the MSCT data, the micro-fpVCT data led to significantly lower FRE values, whereas conventional fpVCT and secondary reconstructed fpVCT data had no advantage in terms of accuracy. For all imaging modalities, there was no relevant visual deviation when targeting defined anatomical points with a navigation probe. fpVCT data are very well suited for electromagnetic navigation at the lateral skull base. The use of titanium screws as fiducial markers turned out to be ideal for comparing different imaging methods. A further evaluation of this approach by a clinical trial is required.
Objective
To evaluate changing trends in patient collectives, age-related patterns of manifestation, and diagnostic pathways of patients with extrapulmonary head and neck tuberculosis (TB), and to provide strategies to fasten diagnosis in these patients.

Case control study.
Methods
A 10-year retrospective analysis of 35 patients diagnosed with extrapulmonary TB in the head and neck at a tertiary university institution from 2009 to 2019, with special focus on the influence of the patient's age on consideration of TB and clinical patterns.
Results
The vast majority of patients younger than 40 years had their origin in countries with high TB burden (P = .0003), and TB was considered very early as a differential diagnosis (P = .0068), while most patients older than 40 years were domestic citizens initially suspected for a malignancy, who more often had an underlying immunosuppressive condition (0.0472). Most frequent manifestations in both groups were the lymph nodes, larynx, and oropharynx. Surprisingly, no differences in the rates of open TB or history of TB infection in the family anamnesis were found.
Conclusion
The two groups of patients found most often are younger patients migrating from regions with high TB burden and elderly domestic patients suffering from immunosuppressive conditions, with the latter often being misdiagnosed as malignancies. TB remains an important but difficult differential diagnosis, due to the initially unspecific symptoms and the great variety in the presentation of manifestations in the head and neck.
Locoregional recurrence is a major reason for therapy failure after surgical resection of head and neck squamous cell carcinoma (HNSCC). The physiological process of postoperative wound healing could potentially support the proliferation of remaining tumor cells. The aim of this study was to evaluate the influence of wound fluid (WF) on the cell cycle distribution and a potential induction of epithelial-mesenchymal transition (EMT). To verify this hypothesis, we incubated FaDu and HLaC78 cells with postoperative WF from patients after neck dissection. Cell viability in dependence of WF concentration and cisplatin was measured by flow cytometry. Cell cycle analysis was performed by flow cytometry and EMT-marker expression by rtPCR. WF showed high concentrations of interleukin (IL)-6, IL-8, IL-10, CCL2, MCP-1, EGF, angiogenin, and leptin. The cultivation of tumor cells with WF resulted in a significant increase in cell proliferation without affecting the cell cycle. In addition, there was a significant enhancement of the mesenchymal markers Snail 2 and vimentin, while the expression of the epithelial marker E-cadherin was significantly decreased. After cisplatin treatment, tumor cells incubated with WF showed a significantly higher resistance compared with the control group. The effect of cisplatin-resistance was dependent on the WF concentration. In summary, proinflammatory cytokines are predominantly found in WF. Furthermore, the results suggest that EMT can be induced by WF, which could be a possible mechanism for cisplatin resistance.
To study the interaction of human pathogens with their host target structures, human tissue models based on primary cells are considered suitable. Complex tissue models of the human airways have been used as infection models for various viral and bacterial pathogens. The Gram-negative bacterium Bordetella pertussis is of relevant clinical interest since whooping cough has developed into a resurgent infectious disease. In the present study, we created three-dimensional tissue models of the human ciliated nasal and tracheo-bronchial mucosa. We compared the innate immune response of these models towards the B. pertussis virulence factor adenylate cyclase toxin (CyaA) and its enzymatically inactive but fully pore-forming toxoid CyaA-AC\(^-\). Applying molecular biological, histological, and microbiological assays, we found that 1 µg/ml CyaA elevated the intracellular cAMP level but did not disturb the epithelial barrier integrity of nasal and tracheo-bronchial airway mucosa tissue models. Interestingly, CyaA significantly increased interleukin 6, interleukin 8, and human beta defensin 2 secretion in nasal tissue models, whereas tracheo-bronchial tissue models were not significantly affected compared to the controls. Subsequently, we investigated the interaction of B. pertussis with both differentiated primary nasal and tracheo-bronchial tissue models and demonstrated bacterial adherence and invasion without observing host cell type-specific significant differences. Even though the nasal and the tracheo-bronchial mucosa appear similar from a histological perspective, they are differentially susceptible to B. pertussis CyaA in vitro. Our finding that nasal tissue models showed an increased innate immune response towards the B. pertussis virulence factor CyaA compared to tracheo-bronchial tissue models may reflect the key role of the nasal airway mucosa as the first line of defense against airborne pathogens.
Causal therapies for the auditory-pathway and inner-ear diseases are still not yet available for clinical application. Regenerative medicine approaches are discussed and examined as possible therapy options. Neural stem cells could play a role in the regeneration of the auditory pathway. In recent years, neural stem and progenitor cells have been identified in the cochlear nucleus, the second nucleus of the auditory pathway. The current investigation aimed to analyze cell maturation concerning cellular calcium activity. Cochlear nuclei from PND9 CD rats were microscopically dissected and propagated as neurospheres in free-floating cultures in stem-cell medium (Neurobasal, B27, GlutaMAX, EGF, bFGF). After 30 days, the dissociation and plating of these cells took place under withdrawal of the growth factors and the addition of retinoic acid, which induces neural cell differentiation. Calcium imaging analysis with BAPTA-1/Oregon Green was carried out at different times during the differentiation phase. In addition, the influence of different voltage-dependent calcium channels was analyzed through the targeted application of inhibitors of the L-, N-, R- and T-type calcium channels. For this purpose, comparative examinations were performed on CN NSCs, and primary CN neurons. As the cells differentiated, a significant increase in spontaneous neuronal calcium activity was demonstrated. In the differentiation stage, specific frequencies of the spontaneous calcium oscillations were measured in different regions of the individual cells. Initially, the highest frequency of spontaneous calcium oscillations was ascertainable in the maturing somata. Over time, these were overtaken by calcium oscillations in the axons and dendrites. Additionally, in the area of the growth cones, an increasing activity was determined. By inhibiting voltage-dependent calcium channels, their expression and function in the differentiation process were confirmed. A comparable pattern of maturation of these channels was found in CN NSCs and primary CN neurons. The present results show that neural stem cells of the rat cochlear nucleus differentiated not only morphologically but also functionally. Spontaneous calcium activities are of great relevance in terms of neurogenesis and integration into existing neuronal structures. These functional aspects of neurogenesis within the auditory pathway could serve as future targets for the exogenous control of neuronal regeneration.
Candida albicans is ubiquitously present, and colonization in the nose and oral cavity is common. In healthy patients, it usually does not act as a pathogen, but in some cases can cause diseases. The influence of C. albicans as a trigger of T cell activation on the pathogenesis of chronic rhinosinusitis (CRS) is controversial, and its exact role is not clear to date. The aim of the present study was to detect and characterize C. albicans-specific CD4+ and CD8+ T cells in patients with CRS, with and without nasal polyps. Tissue and blood samples were collected from patients suffering from chronic rhinosinusitis with (CRSwNP) and without nasal polyps (CRSsNP), and from healthy controls. A peptide pool derived from C. albicans antigen was added to tissue and blood samples. After 6 days, lymphocytes were analyzed by multicolor flow cytometry. Activation was assessed by the intracellular marker Ki-67, and the cytokine secretion was measured. Tissue CD8+ T cells of CRSsNP patients showed a significantly higher proportion of Ki-67+ cells after activation with C. albicans antigen compared to peripheral blood CD8+ T cells. Cytokine secretion in response to C. albicans antigen was similar for all study groups. In this study, C. albicans-specific CD4+ and CD8+ T cells were detected in peripheral blood and mucosal tissue in all study groups. In patients suffering from CRSsNP, C. albicans-specific CD8+ T cells were relatively enriched in the nasal mucosa, suggesting that they might play a role in the pathogenesis of CRSsNP.
In deafness, which is caused by the malfunctioning of the inner ear, an implantation of a cochlear implant (CI) is able to restore hearing. The CI is a neural prosthesis that is located within the cochlea. It replaces the function of the inner hair cells by direct electrical stimulation of the auditory nerve fibers. The CI enables many deaf or severe hearing-impaired people to achieve a good speech perception. Nevertheless, there is a lot of potential for further improvements. Compared to normal-hearing listeners rate pitch discrimination is much worse. Rate pitch discrimination is the ability to distinguish the pitch of two stimuli with two different pulse rates. This ability is important for enjoying music as well as speech perception (in noise). Further, the small dynamic range in electrical hearing (compared to normal-hearing listeners) and therefore the small intensity resolution limits the performance of CI users. Both, rate pitch coding and dynamic range were investigated in this doctoral thesis.
For the first issue, a pitch discrimination task was designed to determine the just-noticeable-difference (JND) in pitch with 200 and 400 pps as reference. Additionally to the default biphasic pulse (single pulse) the experiment was performed with double pulses. The double pulse consists out of two biphasic pulses directly after each other and a small interpulse interval (IPI) in between. Three different IPIs (15, 50, and 150 µs) were tested. The statistical analysis of JNDs revealed no significant effects between stimulation with single-pulse or double-pulse trains.
A follow-up study investigated an alternating pulse train consisting of single and double pulses. To investigate if the 400 pps alternating pulse train is comparable in pitch with the 400 pps single-pulse train, a pairwise pitch comparison test was conducted. The alternating pulse train was compared with single-pulse trains at 200, 300 and 400 pps. The results showed that the alternating pulse train is for most subjects similar in pitch with the 200 pps single-pulse train. Therefore, pitch perception seemed to be dominated by the double pulses within the pulse train.
Accordingly, double pulses with different amplitudes were tested. Based on the facilitation effect, a larger neuronal response was expected by stimulating with two pulses with a short IPI within the temporal facilitation range. In other studies, this effect was shown to be maximal in CIs of the manufacturer Cochlear, with first pulse amplitudes set at or slightly below the electrically evoked compound action potential (ECAP) threshold. The second pulse amplitude did not influence the facilitation effect and therefore could be choose at will. Similarly, this effect was tested in this thesis with CIs of the manufacturer MED-EL. Nevertheless, to achieve a proper signal-to-noise ratio, technical issues had to be addressed like a high noise floor, resulting in incorrect determination of the ECAP threshold. After solving this issues, the maximum facilitation effect was around the ECAP threshold as in the previous study with Cochlear. For future studies this effect could be used in a modified double pulse rate pitch experiment with the first pulse amplitude at ECAP threshold and the second pulse amplitude variable to set the most comfortable loudness level (MCL).
The last study within this thesis investigated the loudness perception at two different loudness levels and the resulting dynamic range for different interphase-gaps (IPG). A larger IPG can reduce the amplitude at same loudness level to save battery power. However, it was unknown if the IPG has an influence on the dynamic range. Different IPGs (10 and 30 µs) were compared with the default IPG (2.1 µs) in a loudness matching experiment. The experiment was performed at the most comfortable loudness level (MCL) of the subject and the amplitude of half the dynamic range (50%-ADR). An upper dynamic range was calculated from the results of MCL and 50%-ADR (therefore not the whole dynamic range was covered). As expected from previous studies a larger IPG resulted in smaller amplitudes. However, the observed effect was larger at MCL than at 50%-ADR which resulted in a smaller upper dynamic range. This is the first time a decrease of this dynamic range was shown.
Einleitung: Die sog. Direct-Drive-Simulation (DDS) bietet Patienten präoperativ eine Klangsimulation des "Vibrant Soundbridge®-Hörens". Ein Floating-Mass-Transducer (FMT) wird auf das Trommelfell oder die rekonstruierte Paukenabdeckung aufgesetzt, worüber den Patienten Testsignale dargeboten werden. Ziel der Arbeit war die Evaluation einer Weiterentwicklung des Tests hin zur DDS-Tonaudiometrie und DDS-Sprachaudiometrie. Bei Bestätigung der ersten Hypothese, soll darüber hinaus geprüft werden, ob eine Hirnstammaudiometrie (BERA) über den DDS durchgeführt werden kann. Ziel der vorliegenden Arbeit ist es Referenzdaten an einer Population Normalhörender zu generieren. Zusammenfassung: Sowohl die DDS-Ton- und Sprachaudiometrie als auch die Ableitung einer BERA über den DDS-FMT sind möglich. Mit der DDS-BERA besteht nun erstmals die Möglichkeit eines objektiven DDS-Tests präoperativ. Die notwendige Einschätzung der Ankopplungsqualität zur korrekten Interpretation der DDS-BERA lässt sich elegant mit Hilfe der DDS-Tonaudiometrie ermitteln.
Objectives: The purpose of this study was to investigate the development of lateralization skills in children who received bilateral cochlear implants (CIs) in sequential operations. Methods: The lateralization skills of 9 children with a mean age of 4.1 years at the first surgery and 5.5 years at the second surgery were assessed at 3 time intervals. Children were assessed with a 3-loudspeaker setup (front, left and right) at 0.9 years (interval I) and 1.6 years (interval II) after the second implantation, and after 5.3 years of bilateral implant use (interval III) with a 9-loudspeaker setup in the frontal horizontal plane between -90° and 90° azimuth. Results: With bilateral implants, a significant decrease in lateralization error was noted between test interval I (45.0°) and II (23.3°), with a subsequent significant decrease at test interval III (4.7°). Unilateral performance with the CI did not improve significantly between the first 2 intervals; however, there was a bias of responses towards the unilateral side by test interval III. Conclusions: The lateralization abilities of children with bilateral CIs develop in a relatively short period of time (1-2 years) after the second implant. Children appear to be able to acquire binaural skills after bilateral cochlear implantation.
Auditory Brainstem Implantation Improves Speech Recognition in Neurofibromatosis Type II Patients

This prospective study aimed to determine speech understanding in neurofibromatosis type II (NF2) patients following implantation of a MED-EL COMBI 40+ auditory brainstem implant (ABI). Patients (n = 32) were enrolled postsurgically. Nonauditory side effects were evaluated at fitting and audiological performance was determined using the Sound Effects Recognition Test (SERT), Monosyllable-Trochee-Polysyllable (MTP) test and open-set sentence tests. Subjective benefits were determined by questionnaire. ABI activation was documented in 27 patients, 2 patients were too ill for testing and 3 patients were without any auditory perception. SERT and MTP outcomes under auditory-only conditions improved significantly between first fitting and 12-month follow-up. Open-set sentence recognition improved from 5% at first fitting to 37% after 12 months. The number of active electrodes had no significant effect on performance. All questionnaire respondents were ‘satisfied' to ‘very satisfied' with their ABI. An ABI is an effective treatment option in NF2 patients with the potential to provide open-set speech recognition and subjective benefits. To our knowledge, the data presented herein is exceptional in terms of the open-set speech perception achieved in NF2 patients.
Objectives: To assess the subjective and objective performance of the new fine structure processing strategy (FSP) compared to the previous generation coding strategies CIS+ and HDCIS. Methods: Forty-six adults with a minimum of 6 months of cochlear implant experience were included. CIS+, HDCIS and FSP were compared in speech perception tests in noise, pitch scaling and questionnaires. The randomized tests were performed acutely (interval 1) and again after 3 months of FSP experience (interval 3). The subjective evaluation included questionnaire 1 at intervals 1 and 3, and questionnaire 2 at interval 2, 1 month after interval 1. Results: Comparison between FSP and CIS+ showed that FSP performed at least as well as CIS+ in all speech perception tests, and outperformed CIS+ in vowel and monosyllabic word discrimination. Comparison between FSP and HDCIS showed that both performed equally well in all speech perception tests. Pitch scaling showed that FSP performed at least as well as HDCIS. With FSP, sound quality was at least as good and often better than with HDCIS. Conclusions: Results indicate that FSP performs better than CIS+ in vowel and monosyllabic word understanding. Subjective evaluation demonstrates strong user preferences for FSP when listening to speech and music.
Objective: To evaluate the safety and effectiveness of round window (RW), oval window (OW), CliP and Bell couplers for use with an active middle ear implant. Methods: This is a multicenter, long-term, prospective trial with consecutive enrollment, involving 6 university hospitals in Germany. Bone conduction, air conduction, implant-aided warble-tone thresholds and Freiburger monosyllable word recognition scores were compared with unaided preimplantation results in 28 moderate-to-profound hearing-impaired patients after 12 months of follow-up. All patients had previously undergone failed reconstruction surgeries (up to 5 or more). In a subset of patients, additional speech tests at 12 months postoperatively were used to compare the aided with the unaided condition after implantation with the processor switched off. An established quality-of-life questionnaire for hearing aids was used to determine patient satisfaction. Results: Postoperative bone conduction remained stable. Mean functional gain for all couplers was 37 dB HL (RW = 42 dB, OW = 35 dB, Bell = 38 dB, CliP = 27 dB). The mean postoperative Freiburger monosyllable score was 71% at 65 dB SPL. The postimplantation mean SRT<sub>50</sub> (speech reception in quiet for 50% understanding of words in sentences) improved on average by 23 dB over unaided testing and signal-to-noise ratios also improved in all patients. The International Outcome Inventory for Hearing Aids (IOI-HA)quality-of-life questionnaire was scored very positively by all patients. Conclusion: A significant improvement was seen with all couplers, and patients were satisfied with the device at 12 months postoperatively. These results demonstrate that an active implant is an advantage in achieving good hearing benefit in patients with prior failed reconstruction surgery.
Introduction: Since 1996, the preferred approach for positioning the active middle-ear implant Vibrant Soundbridge© is a mastoidectomy and a posterior tympanotomy. With this device, placement of the floating mass transducer (FMT) on the long incus process is the standard method for treatment of mild-to-severe sensorineural hearing loss in the case of normal middle-ear anatomy. The aim of this study was to determine the vibrational effectiveness of FMT placement at the short incus process. Materials and Methods: An extended antrotomy and a posterior tympanotomy were performed in 5 fresh human temporal bones. As a control for normal middle-ear function, the tympanic membrane was stimulated acoustically and the vibration of the stapes footplate and the round-window (RW) membrane were (sequentially) measured by laser Doppler vibrometry. Vibration responses for coupling of an FMT to the long incus process (standard coupling) were compared to those for coupling to the short incus process. Results: Apart from narrow frequency bands near 3 and 9 kHz for the stapes footplate and RW membrane, respectively, the velocity responses presented no significant differences between standard coupling of the FMT and coupling to the short incus process. Conclusion: Coupling the FMT to the short incus process may be a viable alternative in cases where the surgical approach is limited to an extended antrotomy. A reliable technique for attachment to the short incus process has yet to be developed.
Neural stem cells (NSCs) have been recently identified in the inferior colliculus (IC). These cells are of particular interest, as no casual therapeutic options for impaired neural structures exist. This research project aims to evaluate the neurogenic potential in the rat IC from early postnatal days until adulthood. The IC of rats from postnatal day 6 up to 48 was examined by neurosphere assays and histological sections. In free-floating IC cell cultures, neurospheres formed from animals from early postnatal to adulthood. The amount of generated neurospheres decreased in older ages and increased with the number of cell line passages. Cells in the neurospheres and the histological sections stained positively with NSC markers (Doublecortin, Sox-2, Musashi-1, Nestin, and Atoh1). Dissociated single cells from the neurospheres differentiated and were stained positively for the neural lineage markers β-III-tubulin, glial fibrillary acidic protein, and myelin basic protein. In addition, NSC markers (Doublecortin, Sox-2, CDK5R1, and Ascl-1) were investigated by qRT-PCR. In conclusion, a neurogenic potential in the rat IC was detected and evaluated from early postnatal days until adulthood. The identification of NSCs in the rat IC and their age-specific characteristics contribute to a better understanding of the development and the plasticity of the auditory pathway and might be activated for therapeutic use.
Das Ziel der vorliegenden Arbeit war die Untersuchung einer Kohorte erwachsener Patienten mit einseitiger Taubheit (single-sided deafness SSD), die ein Cochlea-Implantat erhalten haben. Es sollten Faktoren ermittelt werden, die es ermöglichen, eine Voraussage über die spätere Hörleistung mit Cochlea Implantat treffen zu können.
Der Beobachtungszeitraum umfasste die Jahre 2009 - 2016. Das Patientenkollektiv bestand aus 55 einseitig ertaubten Erwachsenen. Die Sprachverständlichkeit wurde mit dem Freiburger Zahlentest bei 65 dB SPL, dem Freiburger Einsilbertest bei 65 dB SPL und 80 dB SPL, sowie dem HSM-Satztest ohne Störgeräusch bei 65 dB SPL getestet. Es hat sich gezeigt, dass mit steigender Hörerfahrung mit einer Verbesserung der Hörkompetenz zu rechnen ist. Im untersuchten Patientenkollektiv schnitten Patienten mit der Kombination aus einer langen Ertaubungsdauer und einer entzündlichen Ätiologie signifikant schlechter ab als die Vergleichsgruppe.
Purpose
In cancer of unknown primary (CUP), positron emission tomography/computed tomography (PET/CT) with the glucose analog [\(^{18}\)F]FDG represents the standard imaging approach for localization of the malignant primary. Frequently, however, [\(^{18}\)F]FDG PET/CT cannot precisely distinguish between small occult tumors and chronic inflammation, especially in Waldeyer’s tonsillar ring. To improve the accuracy for detecting primary tumors in the Waldeyer’s tonsillar ring, the novel PET tracer [\(^{68}\)Ga]Ga-FAPI-4 for specific imaging of fibroblast activation protein (FAP) expression was used as a more specific target for cancer imaging.
Methods
Eight patients with suspicion of a malignant tumor in Waldeyer’s tonsillar ring or a CUP syndrome were examined. PET/CT scans with [\(^{18}\)F]-FDG and [\(^{68}\)Ga]Ga-FAPI-4 were performed for pre-operative tumor localization. After surgical resection, histopathological and immunohistochemical results were compared to PET/CT findings.
Results
Histopathology revealed a palatine or lingual tonsil carcinoma in all patients. In case of lymph node metastases smaller than 7 mm in size, the [\(^{18}\)F]FDG PET/CT detection rate of cervical lymph node metastases was higher than that of [\(^{68}\)Ga]FAPI PET/CT, while both tracers identified the primary tumors in all eight cases. The size of the primary and the lymph node metastases was directly correlated to the respective FAP expression, as detected by immunohistochemistry. The mean SUVmax for the primary tumors was 21.29 ± 7.97 for \(^{18}\)F-FDG and 16.06 ± 6.29 for \(^{68}\)Ga-FAPI, respectively (p = 0.2). The mean SUVmax for the healthy contralateral tonsils was 8.38 ± 2.45 for [\(^{18}\)F]FDG and 3.55 ± 0.47 for [\(^{68}\)Ga]FAPI (p < 0.001). The SUVmax ratio of [68Ga]FAPI was significantly different from [\(^{18}\)F] FDG (p = 0.03). Mean TBRmax for the [\(^{68}\)Ga]Ga-FAPI-4 tracer was markedly higher in comparison to [\(^{18}\)F]FDG (10.90 vs. 4.11).
Conclusion
Non-invasive imaging of FAP expression by [\(^{68}\)Ga]FAPI PET/CT resulted in a better visual detection of the malignant primary in CUP, as compared to [\(^{18}\)F]FDG imaging. However, the detection rate of lymph node metastases was inferior, presumably due to low FAP expression in small metastases. Nevertheless, by offering a detection method for primary tumors with the potential of lower false positive rates and thus avoiding biopsies, patients with CUP syndrome may benefit from [\(^{68}\)Ga]FAPI PET/CT imaging.
The five tubulin-binding cofactors (TBC) are involved in tubulin synthesis and the formation of microtubules. Their importance is highlighted by various diseases and syndromes caused by dysfunction or mutation of these proteins. Posttranslational modifications (PTMs) of tubulin promote different characteristics, including stability-creating subpopulations of tubulin. Cell- and time-specific distribution of PTMs has only been investigated in the organ of Corti in gerbils. The aim of the presented study was to investigate the cell type-specific and time-specific expression patterns of TBC proteins and PTMs for the first time in murine cochleae over several developmental stages. For this, murine cochleae were investigated at the postnatal (P) age P1, P7 and P14 by immunofluorescence analysis. The investigations revealed several profound interspecies differences in the distribution of PTMs between gerbil and mouse. Furthermore, this is the first study to describe the spatio-temporal distribution of TBCs in any tissue ever showing a volatile pattern of expression. The expression analysis of TBC proteins and PTMs of tubulin reveals that these proteins play a role in the physiological development of the cochlea and might be essential for hearing.
Objective
Cochlear implantation has become a well-accepted treatment option for people with single-sided deafness (SSD) and has become a clinical standard in many countries. A cochlear implant (CI) is the only device which restores binaural hearing. The effect of microphone directionality (MD) settings has been investigated in other CI indication groups, but its impact on speech perception in noise has not been established in CI users with SSD. The focus of this investigation was, therefore, to assess binaural hearing effects using different MD settings in CI users with SSD.
Methods
Twenty-nine experienced CI users with SSD were recruited to determine speech reception thresholds with varying target and noise sources to define binaural effects (head shadow, squelch, summation, and spatial release from masking), sound localization, and sound quality using the SSQ12 and HISQUI19 questionnaires. Outcome measures included the MD settings “natural”, “adaptive”, and “omnidirectional”.
Results
The 29 participants involved in the study were divided into two groups: 11 SONNET users and 18 OPUS 2/RONDO users. In both groups, a significant head shadow effect of 7.4–9.2 dB was achieved with the CI. The MD setting “adaptive” provided a significant head shadow effect of 9.2 dB, a squelch effect of 0.9 dB, and spatial release from masking of 7.6 dB in the SONNET group. No significant summation effect could be determined in either group with CI. Outcomes with the omnidirectional setting were not significantly different between groups. For both groups, localization improved significantly when the CI was activated and was best when the omnidirectional setting was used. The groups’ sound quality scores did not significantly differ.
Conclusions
Adaptive directional microphone settings improve speech perception and binaural hearing abilities in CI users with SSD. Binaural effect measures are valuable to quantify the benefit of CI use, especially in this indication group.
Die chronische Rhinosinusitis (CRS) ist eine entzündliche Erkrankung der Nase und Nasennebenhöhlen mit einem Bestehen von Symptomen über zwölf Wochen im Jahr. In Europa sind über 10% der erwachsenen Bevölkerung von ihr betroffen und leiden unter einer stark verminderten Lebensqualität. Nach dem Phänotyp lassen sich zwei Formen unterscheiden: die chronische Rhinosinusitis mit Polypen (CRScNP) und ohne Polypen (CRSsNP). Meist wird unter europäischen Probanden bei der CRScNP eine hauptsächlich eosinophile Th2- vermittelte und bei der CRSsNP ein gemischtes Profil aus Th1/Th17/Th2-vermittelter Entzündung beschrieben. Doch dies erfasst nicht die Vielfalt dieser Erkrankung. Erst kürzlich konnten Endotypen, basierend auf der Zytokinsekretion, identifiziert werden, die auf ein multifaktorielles Geschehen schließen lassen. Nachdem im Nasensekret von über 90% der CRS Patienten Pilze nachgewiesen werden konnten erscheint es wichtig, deren Rolle in der Pathogenese der CRS zu ergründen. Besonders interessant sind dabei Aspergillus fumigatus und Candida albicans. Als opportunistische Pilze könnten sie im Rahmen der CRS in der Lage sein bei einigen Endotypen durch eine verstärkte Aktivierung von Immunzellen die lokale Entzündungsreaktion aufrecht zu erhalten. Es wurden lokale und periphere CD4+ und CD8+ T-Zellen von Patienten mit CRScNP, CRSsNP und einer gesunden Kontrollgruppe isoliert. Eine Kultivierung mit den Antigenen von C. albicans und A. fumigatus erfolgte in einer Konzentration von 1 µg/ml über 6 Tage. Im Anschluss daran wurden die Zellen für das Durchflusszytometer angefärbt und ihre Aktivität über die Ki67+ Expression gemessen. In den Überständen der Kultivierung wurde durch das Durchflusszytometer mittels Multiplexassay die produzierten Zytokinkonzentrationen der Zellen bestimmt. In dieser Arbeit konnte eine Stimulierung der lokalen und peripheren T-Zellen durch die beiden Antigene gezeigt werden...
Leukoplakien der Stimmlippe können verschiedene histopathologische Diagnosen wie beispielsweise Hyper- oder Parakeratosen, dysplastische Prozesse oder invasive Karzinome des Epithels zugrunde liegen.
Bisher werden klinisch detektierte Leukoplakien nach Probeexzision und histologischer Aufarbeitung anhand verschiedener histomorphologischer Klassifikationssysteme, wie zum Beispiel dem System der WHO oder dem „Ljubljana classification“-System, eingeordnet.
Hinsichtlich der Aussagekraft in Bezug auf die Prognose einer Entwicklung eines invasiven Plattenepithelkarzinoms sind die genannten Systeme jedoch eingeschränkt.
In der Raman-Spektroskopie wird das zu untersuchende Gewebe mit einem Laser bestrahlt. Die beim Durchdringen des Materials entstehende Raman-Streuung ist charakteristisch für verschiedene Moleküle und Bindungen. So kann mithilfe dieser Methodik auf die molekularbiologische Zusammensetzung des Gewebes geschlossen werden.
In dieser Arbeit wurde die Klassifikation verschiedener Dysplasiegrade des Stimmlippenepithels mittels Spektraldaten aus der Raman-Spektroskopie nachvollzogen und erörtert, ob aus den gewonnenen Informationen auf das Risiko der Entstehung eines invasiven Plattenepithelkarzinoms geschlossen werden kann.
Durch den Vergleich der Mittelwertspektren dysplasiefreier, dysplastischer und maligne entarteter Gewebeproben sowie der Methodik einer Hauptkomponentenanalyse kann auf eine unterschiedliche Zusammensetzung der Studiengruppen an Bestandteilen der DNA, Proteine, Lipide und Kohlenhydrate geschlossen werden.
Unter Verwendung einer linearen Diskriminanzanalyse wurden Modelle erstellt, um die Klassifizierbarkeit der verschiedenen Studiengruppen nachzuvollziehen. Außerdem wurden die Modelle auf die Aussagekraft einer Prognose hinsichtlich einer möglichen Karzinomentwicklung überprüft.
Durch diese Modelle können gute Differenzbarkeiten durch die Spektraldaten der einzelnen Studiengruppen untereinander erreicht werden. Zudem kann mit hoher Spezifität, Sensitivität und Genauigkeit prognostiziert werden, ob sich beim jeweiligen Patienten ein invasives Plattenepithelkarzinom der Stimmlippe entwickeln wird.
Um die erlangten Erkenntnisse zu validieren und das Verfahren in den klinischen Alltag zu integrieren, bedarf es jedoch prospektiver Studien.
The human pathogen Bordetella pertussis targets the respiratory epithelium and causes whooping cough. Its virulence factor adenylate cyclase toxin (CyaA) plays an important role in the course of infection. Previous studies on the impact of CyaA on human epithelial cells have been carried out using cell lines derived from the airways or the intestinal tract. Here, we investigated the interaction of CyaA and its enzymatically inactive but fully pore-forming toxoid CyaA-AC– with primary human airway epithelial cells (hAEC) derived from different anatomical sites (nose and tracheo-bronchial region) in two-dimensional culture conditions. To assess possible differences between the response of primary hAEC and respiratory cell lines directly, we included HBEC3-KT in our studies. In comparative analyses, we studied the impact of both the toxin and the toxoid on cell viability, intracellular cAMP concentration and IL-6 secretion. We found that the selected hAEC, which lack CD11b, were differentially susceptible to both CyaA and CyaA-AC–. HBEC3-KT appeared not to be suitable for subsequent analyses. Since the nasal epithelium first gets in contact with airborne pathogens, we further studied the effect of CyaA and its toxoid on the innate immunity of three-dimensional tissue models of the human nasal mucosa. The present study reveals first insights in toxin–cell interaction using primary hAEC.
Adipose-derived stem cells (ASCs) have been extensively studied in the field of stem cell research and possess numerous clinical applications. Cell labeling is an essential component of various experimental protocols and Hoechst 33342 (H33342) represents a cost-effective and easy methodology for live staining. The purpose of this study was to evaluate the labeling of rat ASCs with two different concentrations of H33342 (0.5 μg/mL and 5 μg/mL), with particular regard to usability, interference with cell properties, and potential DNA damage. Hoechst 33342 used at a low concentration of 0.5 μg/mL did not significantly affect cell proliferation, viability, or differentiation potential of the ASCs, nor did it cause any significant DNA damage as measured by the olive tail moment. High concentrations of 5 μg/mL H33342, however, impaired the proliferation and viability of the ASCs, and considerable DNA damage was observed. Undesirable colabeling of unlabeled cocultivated cells was seen in particular with higher concentrations of H33342, independent of varying washing procedures. Hence, H33342 labeling with lower concentrations represents a usable method, which does not affect the tested cell properties. However, the colabeling of adjacent cells is a drawback of the technique.
The goal of this doctoral thesis is to identify appropriate methods for the estimation of connectivity and for measuring synchrony between spike trains from in vitro neuronal networks. Special focus is set on the parameter optimization, the suitability for massively parallel spike trains, and the consideration of the characteristics of real
recordings. Two new methods were developed in the course of the optimization which outperformed other methods from the literature. The first method “Total spiking probability edges” (TSPE) estimates the effective connectivity of two spike trains, based on the
cross-correlation and a subsequent analysis of the cross-correlogram. In addition to the estimation of the synaptic weight, a distinction between excitatory and inhibitory connections is possible. Compared to other methods, simulated neuronal networks could be estimated with higher accuracy, while being suitable for the analysis of massively parallel spike trains. The second method “Spike-contrast” measures the synchrony of parallel spike trains
with the advantage of automatically optimizing its time scale to the data. In contrast to other methods, which also adapt to the characteristics of the data, Spike-contrast is more robust to erroneous spike trains and significantly faster for large amounts of parallel spike trains. Moreover, a synchrony curve as a function of the time scale is generated by Spike-contrast. This optimization curve is a novel feature for the analysis of parallel spike trains.
Patienten mit leicht bis hochgradigen Schallleitungs-, Schallempfindungs- und kombinierten Schwerhörigkeiten werden routinemäßig nach erfolglosem Hörgerätetrageversuch mit aktiven Mittelohrimplantaten versorgt. Aktive Mittelohrimplantate können an verschiedene Strukturen des Mittelohrs angekoppelt werden. Der Ort der Ankopplung ist abhängig vom Hörverlust und der individuellen Physiologie des Mittelohres. Die Hörverbesserung ist dabei stark von der Kopplungseffizienz des Implantatwandlers an die Mittelohrstruktur abhängig.
Aktuell gibt es keine zufriedenstellende Möglichkeit die Kopplungseffizienz intraoperativ zu bestimmen. Daher wird eine objektive Methode eingeführt, um intraoperativ auditorische Hirnstammantworten (BERAs) bei Stimulation über das Implantat abzuleiten. Die Vibrant Soundbrigde® (VSB) wird dabei mit einem Drahtlosüberträger (miniTEK, Signia GmbH, Erlangen) und der Carina®-Aktuator über ein Audiokabel mit der BERA-Anlage verbunden. Die BERA-Anlage überträgt die Stimuli direkt an das Implantat, welches an die Mittelohrstruktur angekoppelt ist. Die BERA-Antworten werden bei der VSB durch einen optimierten VSB-CE-Chirp und beim Carina®-System durch den Standard CE-Chirp evoziert, beginnend bei Pegeln oberhalb der Knochenleitungshörschwelle bis unter die Registrierungsschwelle. Diese Methode kann die intraoperative Integrität des Implantats sowie die Kopplungseffizienz bestimmen, um eine Aussage über den zu erwartenden Hörerfolg treffen zu können. Darüber hinaus kann die versorgte Hörschwelle verwendet werden, um die Anpassung bei Kindern oder schwierigen Fällen zu unterstützen und um eine Hörverschlechterung über die Zeit zu erfassen.
Zusammenfassend, konnte eine Methode zur Bestimmung der intraoperativen Kopplungseffizienz während der Implantation von VSBs und Carinas® etabliert werden. Darüber hinaus werden intraoperative BERA-Daten von 30 VSB- und 10-Carina®-Patienten sowie deren Hörergebnisse gezeigt.
Pulssynchrone Ohrgeräusche – Befunderhebung bei einer standardisiert untersuchten Patientengruppe

Die Arbeit umfasst die Auswertung der klinischen Daten von 108 konsekutiv am Kopf- und Schädelbasis-Zentrum, Klinikum Fulda, zwischen Juli 2013 und September 2015 nachuntersuchten Patienten mit pulssynchronen Ohrgeräuschen. Die Patienten waren gemäß eines einheitlichen, strukturierten und routinemäßigen Diagnose- und Behandlungspfads untersucht worden. Die notwendige Bildgebung umfasste die Computertomographie, die Magnetresonanztomographie sowie die Magnetresonanzangiographie und die Sonographie der Halsgefäße. Eine digitale Subtraktionsangiographie wurde nach strenger Indikationsstellung in ausgewählten Fällen vorgenommen. Ziel war es, zugrundeliegende Ursachen und Diagnosen für das Symptom pulssynchrones Ohrgeräusch festzumachen. Die erhobenen Befunde wurden je nach Plausibilitätsgrad in 4 Gruppen klassifiziert: eindeutig, wahrscheinlich, plausibel, und unklar. Zusätzlich erfolgte die Auswertung der angewandten therapeutischen Behandlungsansätze und ihrer Ergebnisse.
Die häufigsten Ursachen waren ein hochstehender Bulbus der Vena jugularis interna mit 19%, eine Schleife der Arteria cerebelli inferior anterior im inneren Gehörgang mit 15% gefolgt von einer arteriovenösen Fistel mit 10% und Gefäßstenosen mit 9%. Bei ca. 30% unseres Patientenkollektivs konnte keine Quelle für das pulsierende Ohrgeräusch eruiert werden. Bei 29% der Patienten konnte die Ursache als eindeutig klassifiziert werden, bei 25% als plausible Ursache und bei 17% als wahrscheinlich. Bei den insgesamt 14 Patienten, die entweder eine konservative oder eine interventionell/chirurgische Behandlung erhielten, kam es bei 71% zu einer Beseitigung des Ohrgeräusches. 5 von 7 Patienten mit einer a.v. Fistel waren, nach einer erfolgreichen endovaskulären Intervention, postoperativ beschwerdefrei. Eine angioplastische Intervention mittels Stenting führte bei zwei Patienten mit einer ACI Stenose (>70%) zur Beseitigung des Ohrgeräusches. Jeweils ein Patient profitierte von der Behandlung einer Bogengangsdehiszenz, einer intrakraniellen Hypertension sowie einer chirurgischen Sanierung eines Paraganglioms, nach vorgegangener endovaskulärer Embolisation, mit der daraus resultierenden Auslöschung des Ohrgeräusches. Bei den restlichen Patienten persistierte es trotz Behandlung.
Ein pulssynchrones Ohrgeräusch hat viele äußerst unterschiedliche Ursachen. Für eine sachgerechte Evaluation und adäquate Behandlung von Patienten mit pulssynchronem Ohrgeräusch ist eine enge multidisziplinäre Zusammenarbeit unabdingbar. Eine eindeutige Diagnose kann oft nur gestellt werden, wenn alle klinischen sowie alle bildmorphologischen Befunde, die auf einem einheitlichen, routinemäßigen Untersuchungsprotokoll basieren, zusammengetragen und kritisch abgestimmt werden.
Trotz hoher Erfolgsrate und niedriger Komplikationsrate der arteriographischen Embolisation bei der Nasenblutung kommt diese Behandlungsmethode in vielen Institutionen nicht als Standard-Maßnahme im klinischen Kontext zum Einsatz und wird nur als alternativ im Falle eines Scheiterns der chirurgischen arteriellen Clippung in der Blutstillung eingesetzt.
In der vorliegenden retrospektiven Arbeit wurden am Kopf- und Schädelbasiszentrum des Klinikums Fulda die Effizienz und die Sicherheit der perkutanen Embolisation der A. sphenopalatina mit Polyvinylalkohol-Partikeln (150-250 µm) in der Behandlung, der mit konservativen Maßnahmen refraktären Epistaxis untersucht.
Methode: Retrospektive Analyse von 99 Patienten, die von Januar 2001 bis Dezember 2018 bei refraktärer, idiopathisch klassifizierter Epistaxis durch eine arteriographische endovaskuläre Embolisation behandelt wurden.
Ergebnisse: Es wurden innerhalb des Berichtszeitraums insgesamt 100 Embolisationen bei Epistaxis durchgeführt. Nach Ausschluss von 2 Prozeduren mit bekannten Blutungsursachen wurden 98 Embolisationen bei 95 Patienten mit idiopathischer Epistaxis ausgewertet. Trotz technisch erfolgreicher Intervention in 95% der Fälle, war die Blutung in 81,6% der Fälle zu stillen. Leichte Komplikationen traten in 5% der Fälle auf. Schwere Komplikationen dagegen wurden in keinem einzigen Fall erfasst.
Schlussfolgerung: Das Ergebnis dieser Untersuchung bestätigt die Rolle der angiografischen Embolisation der A. sphenopalatina sowohl als einer erfolgreichen, als auch als einer sicheren Methode in der Behandlung der konservativ refraktären Epistaxis.
Aufgrund der geringen Invasivität, hohen Erfolgsrate und niedrigen Komplikationsrate sollte die transkutane Embolisation als Standard-Therapie nach dem Scheitern der konservativen Maßnahmen zur Blutstillung einer Epistaxis zum Einsatz kommen. Die chirurgische Blutstillung durch die arterielle Clippung sollte nur im Falle eines Misserfolgs der Embolisation als Verfahren der zweiten Wahl bei der Blutstillung eingesetzt werden.
Sprachaudiometrische Tests sind subjektive Testverfahren um die Sprachverständlichkeit zu messen. Sie dienen unter anderem zur Indikation für Hörhilfen, zur Verlaufskontrolle und zur Begutachtung. Der deutschsprachige Sprachraum ist von den Freiburger Satztests geprägt. Dennoch gibt es im deutschsprachigen Raum weitere sprachaudiometrische Testverfahren. Der Oldenburger Satztest ist ein moderner Sprachtests, der als Matrixtest syntaktisch korrekte und semantisch inkorrekte Sätze als Sprachmaterial nutzt.
Diese Dissertation untersucht altersabhängige Lerneffekte an einem normalhörenden Probandenkollektiv. Dazu wurden 40 Probanden in homogenen Altersgruppen auf den bereits in der Literatur vielfach beschriebenen Lerneffekt untersucht. Die Dissertation konnte zeigen, dass sich der Lerneffekt in allen Altersgruppen ähnlich verhält. Die Lerneffekte sind damit nicht altersabhängig. Der Lerneffekt zeigte in allen Altersgruppen signifikante Verbesserungen in der fünften durchgeführten Testliste im Vergleich zur ersten durchgeführten Testliste.
Bei Tumoren von Kopf und Hals kann primär oder adjuvant durch Bestrahlung therapiert werden. Die Folgen dieser Behandlung können Xerostomie, Karies, Infektionen, Dysphagie oder Mundgeruch sein. Diese Nebenwirkungen vermindern die Lebensqualität des Patienten. Unterschiedliche Behandlungsansätze haben aufgrund von therapiebedingten Einschränkungen nicht den Weg in den klinischen Alltag gefunden. Eine Alternative zu den vorhandenen Behandlungsansätzen kann das Tissue Engineering sein. Das Ziel einer Normalisierung der Speichelproduktion nach Behandlung soll durch eine implantierbare, künstliche Speicheldrüse erreicht werden. Kann humanes natives Speicheldrüsengewebe der Parotis auf gradientenfreiem dreidimensional aufgebauten Polyurethan wachsen und seine Funktionalität beibehalten?
Humane Parotiszellen wurden von 20 Patienten im Alter von 42 - 90 Jahren durch Operation entnommenen und in Polystyrol-Zellkulturflaschen mit dem Nährmedium BEGM herangezüchtet. Es erfolgte eine 2D-Zellverteilung der reinen Parotiskultur. Zur Kontrolle der Vitalität zwischen den Passagen wurde eine Trypan-Blau Färbung verwendet. Als Trägermaterial der Zellen wurde eine biokompatible, abbaubare Matrix aus ε-Polycaprolacton verarbeitet. Die Übertragung der humanen Parotiszellen wurde mit einer Kleberproteinlösung, bestehend aus den Hauptbestandteilen Aprotinin, Fibrinogen und der Thrombinlösung durchgeführt. 7,14 und 21 Tage nach Aufbringung wurde der Überstand der zeitgleich entnommenen Konstrukte zur Überprüfung des α-Amylase konserviert. Zusätzlich wurden an den 3 Untersuchungstagen Konstrukte für die Anfertigung von histologischen Schnitten, quantitativer PCR, indirekter Immunfluoreszenz und zur Elektronenmikroskopie entnommen. Zur Überprüfung der Funktionalität der angezüchteten Speicheldrüsenzellen wurde das Enzym α-Amylase und das Wasserkanalprotein Aquaporin 5 herangezogen.
Bei der Kultivierung der humanen Speicheldrüsenzellen konnte durch den Vitalitätstest Trypan-Blau Färbung in Kombination mit einer Neubauerzählkammer eine konstant hohe Anzahl an vitalen Zellen bis zur 4. Passage nachgewiesen werden. Durch die Lebend/Tot Färbung auf FDA/EB Basis der Konstrukte über die Untersuchungszeit von 14 Tagen konnte keine Vermehrung von avitalen Zellen mikroskopisch festgestellt werden. Die statistische Auswertung mittels Boxplots des ELISA berechnete für den ersten Untersuchungstag einen Median auf niedrigem Niveau von 4,4 U/l und sank im weiteren Zeitverlauf am Untersuchungstag 21 auf die niedrigsten Median von 2,2 U/l ab. α-Amylase konnte an allen 3 Tagen mittels quantitativer PCR und indirekter Immunfluoreszenz belegt werden. Aquaporin 5 als Funktionsnachweis war in der vorliegenden Studie nicht signifikant durch quantitative PCR beweisbar. Die Rasterelektronenmikroskopie bildete adhärente Zellen in kugeliger Form aus den besiedelten Matrices nach 7 Tagen Kultivierung ab. Durch die Transmissionselektronenmikroskopie konnten Zellen, die Zellfortsätze ausgebildet hatten nach 14 Tagen beobachtet werden. Der Versuch, histologische Schnitte auf Grundlage der Paraffineinbettung oder Kryo-Konservierung zu erzeugen, musste frustran abgeschlossen werden.
Eine Kultivierung von Speicheldrüsenzellen auf einer Matrix aus ε-Polycaprolacton ohne Gradienten ist eingeschränkt umsetzbar. Die Studie konnte zeigen, dass das Wachstum der Zellen auf konstant niedrigem Niveau über den Untersuchungszeitraum von 21 Tagen lag. Der Funktionsnachweis von α-Amylase auf absinkendem niedrigem Niveau sowie fehlender Bestätigung von Aquaporin 5 kann als stationäre Phase des Wachstums interpretiert werden. Zur Verbesserung der Zellentwicklung sollte die besiedelte Matrix zu einem 3D-Zellwachstum anregen. Bei sequenziell entstehender Polarität der Zellen käme es zu einer Verbesserung der Vitalität sowie der vermehrten Ausbildung von α-Amylase und Aquaporin 5. Dies könnte in einer Kombination der Zellkultur aus Parotiszellen mit Kokulturen aus humanen Myoepithelzellen und Parenchymzellen erreicht werden. Sehr gute Ergebnisse des Zellwachstums und der Zellfunktion konnten aktuell in anderen Studien auf der Trägersubstanz Matrigel oder durch Rebesiedelung von dezellularisierten Organen beobachtet werden.
Objectives: The aim of this work is to define critical warning brainstem auditory evoked potential (BAEP) signs as a marker for the postoperative hearing outcome.
Study design: Retrospective study
Setting: Tertiary referral center
Patients: 162 patients who underwent resection of acoustic neuroma via a transtemporal approach with intraoperative monitoring (IOM) at the Department of Otorhinolaryngology, Plastic, Esthetic and Reconstructive Head and Neck Surgery, from January 2011 to December 2017.
Interventions: BAEP was performed in all patients; while intraoperative direct recording of the cochlear nerve function was done in 131 patients.
Main Outcome Measure: postoperative hearing thresholds (Pure tone audiometry).
Results: The most significant risk factor is the permanent loss of wave V as it increases the risk of postoperative hearing loss by 18 times; followed by three-steps increment of the stimulus intensity as it increases the risk by 5.75 times; and finally the response thresholds obtained during the intraoperative direct recording of cochlear nerve function. Each unite increment of the threshold increases the risk of postoperative hearing loss by 6.7%.
Conclusions: We believe that the intraoperative BAEP critical signs during IOM detected in this study can be used as a helpful tool to predict postoperative hearing loss in patients with acoustic neuroma.
Die E-Zigarette gewinnt in den letzten Jahren immer mehr an Popularität. Die Frage der Toxizität ist jedoch noch nicht abschließend geklärt, und es besteht weltweite Unsicherheit bei der Verwendung der E-Zigarette.
Die vorliegende Arbeit untersucht menschliche Nasenschleimhautzellen nach Dampfexposition mit Propylenglykol, einem Hauptbestandteil der Liquide, auf mögliche akute Entzündungsreaktionen, zytotoxische und genotoxische Wirkungen.
Die Nasenschleimhautzellen von 10 Probanden wurden im Air-Liquid-Interface kultiviert und anschließend verschiedenen Konzentrationen von Propylenglykol ausgesetzt. Die Analyse erfolgte unter Verwendung eines Trypanblau-Tests, eines Comet-Assays, eines Mikrokern-Tests und eines IL-6- und IL-8-Sandwich-ELISAs.
Der Trypanblau-Test zeigte keine Reduktion der Vitalität. Im Sandwich-ELISA konnte kein Anstieg der IL-6- und IL-8-Konzentrationen festgestellt werden. Im Comet-Assay zeigte das Olive Tail Moment in allen untersuchten Konzentrationen eine Schädigung im Vergleich zur Negativkontrolle. Es zeigte sich auch eine dosisabhängige Schädigung. Ein Unterschied zwischen der Reinsubstanz und der Negativkontrolle konnte im Mikrokern-Test festgestellt werden.
Es wurden reparierbare Schäden im Comet-Assay gefunden. Im Mikrokern-Test konnten diese nur in der Reinsubstanzkonzentration bestätigt werden. Die E-Zigarette sollte restriktiv verwendet werden, bis Langzeitstudien vorliegen. Darüber hinaus sollten die Hersteller die Inhaltsstoffe der Flüssigkeiten eindeutig angeben.
Proliferationsverhalten epithelialer Tumorzellen in Gegenwart von humanen mesenchymalen Stammzellen

Die Stammzellforschung spielt eine zentrale Rolle in der Optimierung der zielgerichteten und selektiven Tumortherapie durch den angenommenen Tumortropismus von mesenchymalen Stammzellen. Bisher konnten keine eindeutigen Verhaltensmuster mesenchymaler Stammzellen in Gegenwart von Tumorzellen verzeichnet werden. Es gibt sowohl proliferationsfördernde als auch -hemmende Einflüsse. Mögliche Gründe für diese divergierenden Ergebnisse in Bezug auf die Tumorproliferation, im Speziellen auf die Kopf-Hals-Tumorzelllinie FaD, aufzudecken, war das Ziel dieser Arbeit.
We review the role of oscillations in the brain and in the auditory system showing that the ability of humans to distinguish changes in pitch can be explained as a precise analysis of temporal information in auditory signals by neural oscillations. The connections between auditory brain stem chopper neurons construct neural oscillators, which discharge spikes at various constant intervals that are integer multiples of 0.4 ms, contributing to the temporal processing of auditory cochlear output. This is subsequently spatially mapped in the inferior colliculus. Electrophysiological measurements of auditory chopper neurons in different species show oscillations with periods which are integer multiples of 0.4 ms. The constant intervals of 0.4 ms can be attributed to the smallest synaptic delay between interconnected simulated chopper neurons. We also note the patterns of similarities between microcircuits in the brain stem and other parts of the brain (e.g., the pallidum, reticular formation, locus coeruleus, oculomotor nuclei, limbic system, amygdala, hippocampus, basal ganglia and substantia nigra), dedicated to the processing of temporal information. Similarities in microcircuits across the brain reflect the importance of one of the key mechanisms in the information processing in the brain, namely the temporal coupling of different neural events via coincidence detection.
Zinc oxide nanoparticles (ZnO-NPs) are commonly used for industrial applications. Consequently, there is increasing exposure of humans to them. The in vitro analysis of cytotoxicity and genotoxicity is commonly performed under standard cell culture conditions. Thus, the question arises of how the results of genotoxicity and cytotoxicity experiments would alter if human plasma was used instead of cell culture medium containing of fetal calf serum (FCS). Human mesenchymal stem cells (hMSCs) were cultured in human plasma and exposed to ZnO-NPs. A cultivation in expansion medium made of DMEM consisting 10% FCS (DMEM-EM) served as control. Genotoxic and cytotoxic effects were evaluated with the comet and MTT assay, respectively. hMSC differentiation capacity and ZnO-NP disposition were evaluated by histology and transmission electron microscopy (TEM). The protein concentration and the amount of soluble Zn2+ were measured. The cultivation of hMSCs in plasma leads to an attenuation of genotoxic and cytotoxic effects of ZnO-NPs compared to control. The differentiation capacity of hMSCs was not altered. The TEM showed ZnO-NP persistence in cytoplasm in both groups. The concentrations of protein and Zn2+ were higher in plasma than in DMEM-EM. In conclusion, the cultivation of hMSCs in plasma compared to DMEM-EM leads to an attenuation of cytotoxicity and genotoxicity in vitro.
Die fünf Tubulin-bindenden Kofaktoren (TBC) sind an der Tubulinsynthese und der Bildung von Mikrotubuli beteiligt. Ihre Bedeutung wird durch verschiedene Krankheiten und Syndrome hervorgehoben, die durch Funktionsstörungen oder Mutationen dieser Proteine verursacht werden. Posttranslationale Modifikationen (PTMs) von Tubulin fördern verschiedene Eigenschaften, einschließlich stabilitätsfördernder Subpopulationen von Tubulin. Die zell- und zeitspezifische Verteilung der PTMs ist bisher nur im Corti-Organ bei Gerbils untersucht worden. Ziel der vorliegenden Studie war es, die zelltyp- und zeitspezifischen Expressionsmuster von TBC-Proteinen und PTMs erstmals in der murinen Cochlea über mehrere Entwicklungsstadien hinweg zu untersuchen. Dazu wurden murine Cochleae im postnatalen (P) Alter P1, P7 und P14 mittels Immunfluoreszenzanalyse untersucht. Die Untersuchungen zeigten mehrere erhebliche Interspezies-Unterschiede in der Verteilung der PTMs zwischen Gerbil und Maus. Darüber hinaus ist dies die erste Studie, die die räumlich-zeitliche Verteilung von TBCs in einem Gewebe beschreibt, das ein volatiles Expressionsmuster aufweist. Die Expressionsanalyse von TBC-Proteinen und PTMs des Tubulins zeigt, dass diese Proteine eine wichtige Rolle bei der physiologischen Entwicklung der Cochlea spielen und für das Hören essentiell sein könnten.
Adenotonsillectomies are commonly performed procedures and sleep‐disordered breathing is becoming increasingly important as an indication for surgery. Because of the higher risks in patients with obstructive sleep apnoea, the required level of postoperative care for these patients is currently under discussion, and better identification of patients at risk may reduce unnecessary postoperative monitoring. To evaluate the influence of obstructive sleep apnoea, and other risk factors, on peri‐operative complications in children requiring adenotonsillectomy, we performed a retrospective case‐control study that included 1995 patients treated between January 2009 and June 2017. In our analysis, young age (OR 3.8, 95%CI 2.1–7.1), low body weight (OR 2.6, 95%CI 1.5–4.4), obstructive sleep apnoea (OR 2.4, 95%CI 1.5–3.8), pre‐existing craniofacial or syndromal disorders (OR 2.3, 95%CI 1.4–3.8) and adenotonsillectomy, compared with adenoidectomy alone, (OR 7.9, 95%CI 4.7–13.1) were identified as risk factors for complications during or after surgery, p < 0.001. All 13 patients suffering from complications more than 3 h postoperatively had obstructive sleep apnoea plus at least one more of these risk factors. Patients at risk of postoperative complications can therefore be identified by several criteria pre‐operatively, and should be monitored postoperatively using pulse oximetry overnight. For all other patients, postoperative observation on a surgical ward without extra monitoring is sufficient. Admission to paediatric intensive care should be reserved for patients suffering serious intra‐operative complications.
