Melanome stellen die gefährlichste Form von Hautkrebs mit der höchsten Mortalitätsrate dar. Der Transformation normaler Melanozyten zu malignen Melanomen liegen komplexe molekulare und biochemische Veränderungen zu Grunde. Im Xiphophorus-Melanom-Modell ist die onkogene Rezeptortyrosinkinase "Xiphophorus melanoma receptor kinase" (Xmrk) der alleinige Auslöser der Melanominitiation und -progression. Die Aufklärung der Xmrk-vermittelten Signaltransduktion kann zum besseren Verständnis von Ereignissen, die auch bei der humanen Melanomentwicklung eine Rolle spielen, beitragen. In der vorliegenden Arbeit wurde mit Hilfe der Microarray-Technologie die Regulation der Genexpression durch Xmrk analysiert. Zu den nach Rezeptoraktivierung am stärksten herabregulierten Genen gehörten "son of sevenless homolog 1" (Sos1) und "ubiquitin-conjugating enzyme E2I" (Ube2i); stark hochreguliert waren "early growth response 1" (Egr1), "cysteine-rich protein 61" (Cyr61), "dual-specificity phosphatase 4" (Dusp4), "fos-like antigen 1" (Fosl1), "epithelial membrane protein" (Emp1), Osteopontin (Opn), "insulin-like growth factor binding protein 3" (Igfbp3) und "tumor-associated antigen L6" (Taal6). Die für die Regulation dieser Gene verantwortlichen Signalwege wurden durch die Anwendung von niedermolekularen Inhibitoren und siRNA identifiziert, wobei für die SRC-Kinase FYN eine zentrale Bedeutung bei der Xmrk-abhängigen Regulation der Genexpression festgestellt wurde. Darüber hinaus wurde die Expression der Gene in humanen Melanomzelllinien im Vergleich zu normalen humanen Melanozyten untersucht. Als besonders vielversprechende Kandidaten stellten sich dabei DUSP4 und TAAL6 heraus, deren Rolle in der humanen Melanominduktion und -progression Gegenstand zukünftiger Studien sein wird. In einem anderen Ansatz zur Aufklärung des Signalnetzwerkes sollten Zielproteine von Xmrk durch Protein-Protein-Interaktionsstudien mit Hilfe des Split-Ubiquitin-Systems ermittelt werden. Aufgrund ungünstiger Expressions- oder Faltungseigenschaften von Xmrk in diesem System war es aber nicht möglich, den Rezeptor als Köderprotein einzusetzen. Das für die Xmrk-vermittelte Melanomentstehung zentrale Protein FYN konnte jedoch als Köder etabliert und seine Wechselwirkung mit der Tyrosinkinase FAK analysiert werden. Es wurde gezeigt, dass der phosphorylierte Tyrosinrest an Position 397 von FAK für die Interaktion einer N-terminal trunkierten FAK-Variante mit FYN notwendig ist und dass diese Phosphorylierung in Hefe gewährleistet zu sein scheint. Die Suche nach neuen Interaktionspartnern von FYN mittels der Split-Ubiquitin-Technologie könnte Einblicke in weitere FYN-abhängige Ereignisse bieten, die zur Aufklärung seiner zentralen Rolle bei der Tumorentstehung dienen könnte.
Molecular-based subclassifications of breast cancer are important for identifying treatment options and stratifying the prognosis in breast cancer. This study aimed to assess the prognosis relative to disease-free survival (DFS) and overall survival (OS) in patients with triple-negative breast cancer (TNBC) and other subtypes, using a biomarker panel including cytokeratin 5 (CK5), cluster of differentiation 117 (CD117), and epidermal growth factor receptor (EGFR). This cohort–case study included histologically confirmed breast carcinomas as cohort arm. From a total of 894 patients, 572 patients with early breast cancer, sufficient clinical data, and archived tumor tissue were included. Using the immunohistochemical markers CK5, CD117, and EGFR, two subgroups were formed: one with all three biomarkers negative (TBN) and one with at least one of those three biomarkers positive (non-TBN). There were significant differences between the two biomarker subgroups (TBN versus non-TBN) in TNBC for DFS (p = 0.04) and OS (p = 0.02), with higher survival rates (DFS and OS) in the non-TBN subgroup. In this study, we found the non-TBN subgroup of TNBC lesions with at least one positive biomarker of CK5, CD117, and/or EGFR, to be associated with longer DFS and OS.
SphK1 is known to play a role in tumor progression, resistance to radiochemotherapy, and migration patterns. As the overall survival rates of squamous cell carcinoma of the head and neck (HNSCC) remain poor due to limitations in surgery and irradiation and chemotherapy resistance, SphK1 is an important enzyme to investigate. The purpose of this study was to elucidate the impact of SphK1 on irradiation efficacy of HNSCC in-vitro with emphasis on EGFR signaling. By immunhistochemical staining we found a positive correlation between EGFR and SphK1 expression in patient specimens. In colony formation assays irradiation sensitive cell lines showed a poor response to cetuximab, an EGFR inhibitor, and SKI-II, a SphK1 inhibitor, and vice versa. In irradiation sensitive cells an enhanced reduction of cell migration and survival was found upon simultaneous targeting of EGFR and SphK1. In the present study, we elucidated a linkage between the two signaling pathways with regard to the efficacy of cetuximab treatment and the impact on the migration behavior of tumor cells. We investigated the biological impact of inhibiting these pathways and examined the biochemical implications after different treatments. An understanding of the processes involved could help to improve the treatment of patients with HNSCC.
Epidermal growth factor receptor (EGFR) and its ligand heparin-binding EGF-like growth factor (HB-EGF) sustain endothelial cell proliferation and angiogenesis in solid tumors, but little is known about the role of HB-EGF–EGFR signaling in bone marrow angiogenesis and multiple myeloma (MM) progression. We found that bone marrow endothelial cells from patients with MM express high levels of EGFR and HB-EGF, compared with cells from patients with monoclonal gammopathy of undetermined significance, and that overexpressed HB-EGF stimulates EGFR expression in an autocrine loop. We also found that levels of EGFR and HB-EGF parallel MM plasma cell number, and that HB-EGF is a potent inducer of angiogenesis in vitro and in vivo. Moreover, blockade of HB-EGF–EGFR signaling, by an anti-HB-EGF neutralizing antibody or the EGFR inhibitor erlotinib, limited the angiogenic potential of bone marrow endothelial cells and hampered tumor growth in an MM xenograft mouse model. These results identify HB-EGF–EGFR signaling as a potential target of anti-angiogenic therapy, and encourage the clinical investigation of EGFR inhibitors in combination with conventional cytotoxic drugs as a new therapeutic strategy for MM.
Die Rolle der Hirnrinde als Zentrum komplexer Funktionen wie Lernen und Ge-dächtnis wird nicht zuletzt durch deren komplexe, in Schichten organisierte Architek-tur ermöglicht. Von entscheidender Bedeutung ist die präzise Positionierung von Nervenzellen, die im Laufe der Embryonalentwicklung in der Ventrikularzone (VZ) geboren werden und anschließend in radialer Richtung zu ihrem Bestimmungsort wandern. Die Funktion des Neurotrophin-Rezeptors TrkB an der Entwicklung des zerebralen Kortex war Gegenstand dieser Arbeit. Am Tag 12,5 der Embryonalentwicklung konnte die Expression von TrkB so-wohl in den Zellen der VZ als auch in neu geborenen Neuronen der Präplatte nach-gewiesen werden. Die Phosphorylierung des Rezeptors erfolgte dabei unabhängig von den beiden Liganden BDNF und NT-3. Ebenso führten BDNF oder NT-3 zu keiner zellulären Antwort in isolierten kortikalen Vorläuferzellen, wohingegen die Stimulation mit EGF eine Phosphorylierung von TrkB an der PLCγ- und der Shc-Bindungsstelle hervorrief. Durch pharmakologische Inhibition und die Überexpression dominant negativer Src-Mutanten konnte die Beteiligung des EGF-Rezeptors und zweier neuronal exprimierter Src-Kinasen, cSrc und Fyn, an dieser Transaktivierung von TrkB durch EGF gezeigt werden. Durch die Zugabe von EGF kam es im Zuge der Aktivierung von TrkB auch zur Umverteilung des Rezeptors von intrazellulären Kompartimenten zur Zellmem-bran. Die Retention des Rezeptors im Zytoplasma wurde über post-translationelle Modifikation reguliert. Die Verhinderung von N-Glykosylierung durch Tunicamycin-Behandlung kortikaler Vorläuferzellen führte zur Exposition von TrkB an der Zellober-fläche und konnte so Responsivität gegenüber BDNF herstellen. Die physiologische Bedeutung einer Transaktivierung von TrkB durch EGF wurde durch das Fehlen der TrkB-Aktivierung in EGFR KO-Mäusen am Embryonal-tag 12,5 gezeigt. Dies hatte eine fehlerhafte Positionierung kortikaler Nervenzellen zum Zeitpunkt E15,5 zur Folge. Anhand eines Migrationsassays konnte schließlich gezeigt werden, dass die EGF-induzierte Wanderung kortikaler Vorläuferzellen in vitro mit einer asymmetrischen Translokation von TrkB einhergeht. Über die Transaktivierung von TrkB in frühen Phasen der Kortexentwicklung spielt EGF eine wichtige Rolle bei der Induktion neuronaler Differenzierung und ist an der Regulation der Wanderung postmitotischer Neurone in der Hirnrinde beteiligt.
Critical illness like sepsis, shock, and intestinal bowel disease are one of the leading causes of morbidity and mortality in the US and around the world. At present, studies to define new therapeutic interventions that can protect tissues and cells against injury and attenuate inflammation are fields of intense investigation. While research over the past decade has clearly identified GLN as a vital stress substrate facilitating cellular survival following injury, the initiation steps in GLN’s cytoprotective molecular mechanism still remain elusive. Previously published work suggested that stabilization of ECM proteins and activation of ECM receptor osmosignaling may play a central role in the orchestration of many cellular pathways following stress. Thus, I hypothesized that preservation of ECM protein and EGFR levels as well as ECM receptor signaling play key roles in the molecular mechanisms underlying GLN’s protection against thermal injury in the intestine. I was able to confirm via Western blotting and by using silencing RNA against FN, Ntn-1, EGFR, and their negative controls, that GLN-mediated preservation of FN, Ntn-1, and EGFR levels is critical in GLN’s protection against hyperthermia in IEC-6 cells. By using a selective FN-Integrin interaction inhibitor GRGDSP, its negative control peptide GRGESP, and Src-kinase inhibitor PP2, I showed that FN-Integrin signaling and Src-kinase activation are essential in GLN-mediated protection in the intestine. This applied to EGFR signaling as demonstrated using the EGFR tyrosine kinase inhibitor AG1478. In addition to GRGDSP and AG1478, ERK1/2 inhibitors PD98059 and UO126 as well as the p38MAPK inhibitor SB203580 revealed that GLN is protective by activating ERK1/2 and dephosphorylating p38MAPK via FN-Integrin and EGFR signaling. However, GLN-mediated PI3-K/Akt/Hsp70 activation seems to occur independently of FN-Integrin and EGFR signaling as indicated by Western blots as well as experiments using the PI3-K inhibitor LY294002, GRGDSP, and AG1478. The results showed that GLN activates cell survival signaling pathways via integrins as well as EGFRs after hyperthermia. Moreover, I found that GLN-mediated preservation of FN expression after HS is regulated via PI3-K signaling. Whether GLN-mediated PI3-K signaling happens simultaneously to FN-Integrin and EGFR signaling or whether PI3-K signaling coordinates FN-Integrin and EGFR signaling needs to be investigated in future studies. Further, experiments with PD98059 and GRGDSP revealed that ERK1/2 assists in mediating transactivation of HSF-1 following HS. This leads to increases in Hsp70 expression via FN-Integrin signaling, which is known to attenuate apoptosis after thermal injury. Fluorescence microscopy results indicated that HS and GLN regulate cell are size changes and the morphology of F-actin via FN-Integrin signaling. Experiments using GRGDSP and GRGESP showed that GLN enhances cellular survival via FN-Integrin signaling in a manner that does not require increased intracellular GLN concentrations (as quantified using LC-MS/MS). In summary, my thesis work gives new and potentially clinically relevant mechanistic insights into GLN-mediated molecular cell survival pathways. These results warrant clinical translation to assess if clinical outcome of critically ill patients suffering from gastrointestinal diseases can be improved by GLN treatment and/or by targeting the molecular pathways found in my studies.
Fish of the genus Xiphophorus belong to the oldest animal models in cancer research. The oncogene responsible for the generation of spontaneous aggressive melanoma encodes for a mutated epidermal growth factor receptor (Egfr) and is called xmrk for Xiphophorus melanoma receptor kinase. Xmrk constitutive activation mechanisms and subsequent signaling pathways have already been investigated and charaterized but it is still unknown if Egfr ligands may also play a role in Xmrk-driven melanoma formation. To investigate the potential role of Egfr ligands in Xmrk-driven melanoma, I firstly analyzed the evolution of teleost and tetrapod Egfr/Egfr ligand systems. I especially focused on the analysis on the medaka fish, a closely related species to Xiphophorus, for which the whole genome has been sequenced. I could identify all seven Egfr ligands in medaka and could show that the two teleost-specific Egfr copies of medaka display dissimilar expression patterns in adult tissues together with differential expression of Egfr ligand subsets, arguing for subfunctionalization of receptor functions in this fish. Our phylogenetic and synteny analyses supported the hypothesis that only one gene in the chordate ancestor gave rise to the diversity of Egfr ligands found in vertebrate genomes today. I also could show that the Egfr extracellular subdomains implicated in ligand binding are not evolutionary conserved between tetrapods and teleosts, making the use of heterologous ligands in experiments with fish cells debatable. Despite its well understood and straight-forward process, Xmrk-driven melanomagenesis in Xiphophorus is problematic to further investigate in vivo. Our laboratory recently established a new melanoma animal model by generating transgenic mitf::xmrk medaka fishes, a Xiphophorus closely related species offering many more advantages. These fishes express xmrk under the control of the pigment-cell specific Mitf promoter. During my PhD thesis, I participated in the molecular analysis of the stably transgenic medaka and could show that the Xmrk-induced signaling pathways are similar when comparing Xiphophorus with transgenic mitf::xmrk medaka. These data together with additional RNA expression, protein, and histology analyses showed that Xmrk expression under the control of a pigment cell-specific promoter is sufficient to induce melanoma in the transgenic medaka, which develop very stereotyped tumors, including uveal and extracutaneous melanoma, with early onset during larval stages. To further investigate the potential role of Egfr ligands in Xmrk-driven melanoma, I made use of two model systems. One of them was the above mentioned mitf::xmrk medaka, the other was an in-vitro cell culture system, where the EGF-inducible Xmrk chimera HERmrk is stably expressed in murine melanocytes. Here I could show that HERmrk activation strongly induced expression of amphiregulin (Areg) and heparin-binding EGF-like growth factor (Hbegf) in melanocytes. This regulation was dependent on the MAPK and SRC signaling pathways. Moreover, upregulation of Adam10 and Adam17, the two major sheddases of Egfr ligands, was observed. I also could demonstrate the functionality of the growth factors by invitro analyses. Using the mitf::xmrk medaka model I could also show the upregulation of a subset of ligand genes, namely egf, areg, betacellulin (btc) and epigen (epgn) as well as upregulation of medaka egfrb in tumors from fish with metastatic melanoma. All these results converge to support an Xmrk-induced autocrine Egfr ligand loop. Interestingly, my in-vitro experiments with conditioned supernatant from medaka Egf- and Hbegf-producing cells revealed that not only Xiphophorus Egfrb, but also the pre-activated Xmrk could be further stimulated by the ligands. Altogether, I could show with in-vitro and in-vivo experiments that Xmrk is capable of inducing a functional autocrine Egfr ligand loop. These data confirm the importance of autocrine loops in receptor tyrosine kinase (RTK)-dependent cancer development and show the possibility for a constitutively active RTK to strengthen its oncogenic signaling by ligand binding.
Receptor tyrosine kinases (RTK) are rarely mutated in cutaneous melanoma, but the expression and activation of several RTK family members are associated with a proinvasive phenotype and therapy resistance. Epidermal growth factor receptor (EGFR) is a member of the RTK family and is only expressed in a subgroup of melanomas with poor prognosis. The insight into regulators of EGFR expression and activation is important for the understanding of the development of this malignant melanoma phenotype. Here, we describe that the transcription factor NRF2, the master regulator of the oxidative and electrophilic stress response, mediates the expression and activation of EGFR in melanoma by elevating the levels of EGFR as well as its ligands EGF and TGFα. ChIP sequencing data show that NRF2 directly binds to the promoter of EGF, which contains a canonical antioxidant response element. Accordingly, EGF is induced by oxidative stress and is also increased in lung adenocarcinoma and head and neck carcinoma with mutationally activated NRF2. In contrast, regulation of EGFR and TGFA occurs by an indirect mechanism, which is enabled by the ability of NRF2 to block the activity of the melanocytic lineage factor MITF in melanoma. MITF effectively suppresses EGFR and TGFA expression and therefore serves as link between NRF2 and EGFR. As EGFR was previously described to stimulate NRF2 activity, the mutual activation of NRF2 and EGFR pathways was investigated. The presence of NRF2 was necessary for full EGFR pathway activation, as NRF2-knockout cells showed reduced AKT activation in response to EGF stimulation compared to controls. Conversely, EGF led to the nuclear localization and activation of NRF2, thereby demonstrating that NRF2 and EGFR are connected in a positive feedback loop in melanoma. In summary, our data show that the EGFR-positive melanoma phenotype is strongly supported by NRF2, thus revealing a novel maintenance mechanism for this clinically challenging melanoma subpopulation.
In a variety of established tumour cell lines, but also in primary mammary epithelial cells metalloprotease-dependent transactivation of the EGFR, and EGFR characteristic downstream signalling events were observed in response to stimulation with physiological concentrations of GPCR agonists such as the mitogens LPA and S1P as well as therapeutically relevant concentrations of cannabinoids. Moreover, this study reveals ADAM17 and HB-EGF as the main effectors of this mechanism in most of the cancer cell lines investigated. However, depending on the cellular context and GPCR agonist, various different members of the ADAM family are selectively recruited for specific ectodomain shedding of proAR and/or proHB-EGF and subsequent EGFR activation. Furthermore, biological responses induced by LPA or S1P such as migration in breast cancer and HNSCC cells, depend on ADAM17 and proHB-EGF/proAR function, respectively, suggesting that highly abundant GPCR ligands may play a role in tumour development and progression. Moreover, EGFR signal transactivation could be identified as the mechanistic link between cannabinoid receptors and the activation of mitogen activated protein kinases (MAPK) ERK1/2 as well as pro-survival Akt/PKB signalling. Depending on the cellular context, cannabinoid-induced signal cross-communication was mediated by shedding of proAmphiregulin and/or proHB-EGF by ADAM17. Most importantly, our data show that concentrations of THC comparable to those detected in the serum of patients after THC administration accelerate proliferation of cancer cells instead of apoptosis and thereby may contribute to cancer progression in patients.
Klassischerweise ist der Aldosteron-gebundene MR an der Regulation des Blutdruckes und des Wasser-Elektrolyt-Haushaltes beteiligt. Neuere klinische Studien zeigen allerdings, dass Aldosteron auch an pathophysiologischen Remodelingprozessen im kardiovaskulären und renalen System mitwirkt. Die zugrundeliegenden Mechanismen sind noch weitgehend unbekannt. Der EGFR ist ein Wachstumsfaktor und heterologer Signaltransduktor für G-Protein-gekoppelte Rezeptoren von beispielsweise Angiotensin II, Phenylephrin und Endothelin-1. In der Literatur gibt es Hinweise für eine Interaktion zwischen den Signaltransduktionswegen von Aldosteron/MR und EGFR. So können Mineralocorticoide nach zerebraler Ischämie zu einem vermehrten vaskulären Remodeling und einem Anstieg der EGFR-mRNA-Konzentration führen und außerdem eine EGF-induzierte Vasokonstriktion verstärken. Daher wäre eine mögliche Erklärung für die pathophysiologische Wirkung von Aldosteron eine Induktion der EGFR-Expression mit vermehrter Wirksamkeit von vasoaktiven Peptiden. Um diese Hypothese zu überprüfen untersuchten wir in verschiedenen Modellsystemen, ob Aldosteron die EGFR-Proteinexpression erhöht. Dies war sowohl im heterologen CHO-Expressionsystem also auch in MR-exprimierenden Zelllinien und Primärkulturen der Fall. Auch in adrenalektomierten Ratten mit osmotischen Minipumpen bestätigte sich die Aldosteron-induzierte EGFR-Expression in der Aorta, im linken Herzen und der Niere. Über den eng verwandten Glucocorticoidrezeptor ließ sich keine EGFR-Expressionssteigerung auslösen, so dass es sich um einen MR-spezifischen Effekt handelt. Zur Charakterisierung des zugrundeliegenden molekularen Mechanismus, der besonders für therapeutische Interventionen von Interesse ist, wurde die Promotoraktivität des EGFR untersucht. Es zeigte sich bei Aldosteroninkubation eine gesteigerte EGFR-Promotoraktivität im Reporter-Gen-Assay. Die beteiligten Promotoranteile konnten mit Deletionskonstrukten auf zwei DNA-Fragmente eingegrenzt werden. Von Seiten des MR ist die A/B-Domäne für die Interaktion bedeutend, denn ein trunkierter MR mit den Domänen C, D, E und F genügt nicht, um den EGFR-Promoter vollständig zu aktivieren. Um Hinweise für die physiologische und pathophysiologische Bedeutung der Interaktion zwischen MR und EGFR zu erhalten, untersuchten wir sowohl den Einfluß auf die Bildung von Extrazellulärmatrix in glatten Gefäßmuskelzellen als auch auf die Natriumresorption im Sammelrohr der Niere. Als Anhaltspunkt für die vermehrte Bildung von extrazellulärer Matrix wie sie bei Remodelingprozessen vorkommt, quantifizierten wir die Fibronektinsekretion in glatten Muskelzellen der humanen Aorta (HAoSMC). Nach Aldosteroninkubation und besonders bei Koinkubation mit EGF zeigte sich eine vermehrte Fibronektinsekretion ins Medium, die sich durch Hemmer der EGFR-Kaskade normalisieren ließ. Dies unterstützt die Hypothese, dass die Aldosteron-EGFR-Interaktion an der Entstehung von Remodelingprozessen im kardiovaskulären und renalen System beteiligt ist. Neben einem Einfluss auf die Entstehung pathophysiologischer Prozesse im kardiovaskulären und renalen System kommt es über eine Aldosteron-induzierte EGFR-Expression im Sammelrohr der Niere auch zu physiologischen Effekten, nämlich einer Hemmung der Natriumresorption. Diese wirkt der klassischerweise durch Aldosteron vermittelten vermehrten Natriumresoprtion über den epithelialen Natriumkanal (ENaC) entgegen und könnte daher als negative Feedbackschleife Dauer und Ausmaß der Aldosteron-induzierten Natriumresorption limitieren. Zusätzlich zu den klassischen genomischen Wirkungen zeigen Steroide nicht-genotrope Effekte. Beim Aldosteron führen diese MR- und EGFR-vermittelt zu einer Aktivierung der ERK1/2- und JNK-1/2-Kinasen. Die nicht-genotrope Aldosteron-induzierte ERK-Aktivierung ist ferner durch c-Src-Inhibitoren hemmbar und führt zu einer Stimulation der Kerntranslokation des MR. Nicht-genotrope Effekte können folglich unter Beteiligung der EGFR-Signalkaskade die genomischen modulieren. Aldosteron führt ebenfalls zu einem Anstieg der zytosolischen Calciumkonzentration, allerdings ist dieser Effekt unabhängig vom MR. Hieraus folgt, dass die nicht-genotropen Effekte teilweise MR-vermittelt und teilweise MR-unabhängig sind. Insgesamt konnte also auf verschiedenen Ebenen eine Interaktion zwischen Aldosteron/MR und der EGFR-Signalkaskade gezeigt werden, mit Hinweisen für eine Bedeutung bei sowohl physiologischen als auch pathophysiologische Vorgängen.
Das Nebennierenrindenkarzinom ist ein seltener maligner, epithelialer Tumor mit einer jährlichen Inzidenz in Deutschland von 2 pro 1 Million Einwohnern. Es ist für etwa 0,2% aller Krebstodesfälle verantwortlich. Die Seltenheit der Erkrankung erschwert die Entwicklung wirksamer Therapieansätze erheblich. Erst in letzter Zeit gelang es, größere Patientenkohorten in kontrollierten Studien zu behandeln (Allolio et al. 2006) und größere Fallzahlen an Tumorgewebe wissenschaftlichen Untersuchungen zuzuführen. Erstmalig besteht damit auch die Möglichkeit, die Expression eventuell relevanter Proteine für moderne Therapiekonzepte auch an großen Fallzahlen zu untersuchen. Bislang liegen nur sehr lückenhafte diesbezügliche Studien vor. In der vorliegenden Arbeit wurde die Expression einiger möglicherweise therapeutisch relevanter Proteine mittels immunhistochemischer Analyse unter Zuhilfenahme der Tissue Microarray-(TMA)-Technik in einer großen Serie von 104 NNR-Ca untersucht und mit der von Nebennierenadenomen und normalem Nebennierengewebe verglichen. Insgesamt 9 verschiedene Proteine wurden immunhistochemisch dargestellt: Als wichtigster Befund wurde eine Überexpression der Alpha-methylacyl-CoA Racemase (AMACR) in 89% der untersuchten NNR-Ca festgestellt. Interessanterweise zeigte auch die Mehrzahl der untersuchten Nebennierenadenome (80%) zumindest eine schwache AMACR-Reaktivität, während die mituntersuchten normalen Nebennieren negativ blieben. Diese bislang nicht beschriebene Beobachtung gleicht den bekannten Befunden z.B. in der Prostata, in der sowohl die dort entstehenden Adenokarzinome, als auch deren Vorläuferläsionen (PIN) eine AMACR-Expression zeigen. EGFR, ein Tyrosinkinaserezeptor, auf den der bereits etablierte therapeutische Antikörper Cetuximab (Erbitux®) zielt, wurde in 49% der untersuchten NNR-Ca verstärkt exprimiert, so dass eine gegen den EGFR gerichtete Therapie in ausgewählten Fällen eine Behandlungsoption zu eröffnen scheint. Die Suche nach Mutationen in diesem potentiellen Zielprotein könnte weitere Ansatzpunkte für Wirkstoffe aufzeigen, Studien gibt es dazu bislang keine. Ähnliche Ergebnisse fanden sich in dieser Studie für VEGF, das von 19% der untersuchten NNR-Ca mäßig oder stark exprimiert wurde. Auch hier könnte eine entsprechende Therapie mit einem bereits kommerziell erhältlichen spezifischen Antikörper (Bevacizumab/Avastin®) in einem Teil der Fälle eine Behandlungsoption, ggf. auch als supportive Maßnahme, eröffnen. Angesichts der Expression von VEGF in nur rund einem Drittel der Fälle, wäre hier aber eine vorangehende Bestimmung des Expressionsstatus am Tumorgewebe unerläßlich. Weitere Untersuchungen sollten auch den VEGF-Rezeptor als möglichen Ansatzpunkt neuer Wirkstoffe, wie z.B. der Multi-Tyrosinkinase-Inhibitor Sunitinib [SU11248; SUTENTTM] (Le Tourneau et al., 2007), erforschen. Im Gegensatz dazu wurde eine wesentliche Expression weiterer untersuchter Proteine (CD117/C-kit, Her-2/neu, Östrogen- und Progesteronrezeptor) in NNR-Ca in dieser Studie nicht in einem Maße gefunden, welches in einer signifikanten Zahl von Fällen eine Therapieoption eröffnen würde. Eine immunhistochemisch bestimmbare Expression von p53, als Surrogatmarker für ein mutiertes p53-Gen wurde in 85% der untersuchten NNR-Ca nachgewiesen. Dies weist auf eine zentrale Bedeutung von p53-Mutationen in der Genese von NNR-Ca hin. (Reincke et al., 1994; Gicquel et al., 2001; Barzon et al.I, 2001; Soon et al., 2008) Auch die Proliferationsfraktion, bestimmt als Ki67-Index, war in den untersuchten Karzinomen gegenüber den Adenomen und den mitgeführten normalen Nebennieren deutlich erhöht, was das aggressive Wachstumspotential der Mehrzahl der NNR-Ca wiederspiegelt.
The Role of Sphingosine 1-phosphate and S1PR1-3 in the Pathophysiology of Meningococcal Meningitis

Neisseria meningitidis (N. meningitidis) is an obligate human pathogen which causes live-threatening sepsis and meningitis. The fatality rate after meningococcal infection is high and surviving patients often suffer from severe sequelae. To cause meningitis, N. meningitidis must overcome the endothelium of the blood-brain barrier. The bacterium achieves this through the interaction with endothelial surface receptors leading to alternations of the cellular metabolism and signaling, which lastly results in cellular uptake and barrier traversal of N. meningitidis. Sphingosine 1-phosphate (S1P) is a lipid mediator that belongs to the class of sphingolipids and regulates the integrity of the blood-brain barrier through the interaction with its cognate receptors S1P receptors 1-3 (S1PR1-3).
In this study, high performance liquid chromatography coupled with mass spectrometry (LC-MS/MS) was used to generate a time-resolved picture of the sphingolipid metabolism in a brain endothelial cell line (hCMEC/D3) upon meningococcal infection. Among various changes, S1P was elevated in the cellular compartment as well as in the supernatant of infected hCMEC/D3s. Analysis of mRNA expression in infected hCMEC/D3s with quantitative real-time polymerase chain reaction (RT-qPCR) revealed that the increase in S1P could be attributed to the enhanced expression of the S1P-generating enzyme sphingosine kinase 1 (SphK1). Antibody-based detection of SphK1 protein or phosphorylation at SphK1 residue Serine 225 in hCMEC/D3 plasma membrane fractions via Western Blot revealed that N. meningitidis also induced SphK1 phospho-activation and recruitment to the plasma membrane. Importantly, recruitment of SphK1 to the plasma membrane increases the probability of substrate encounter, thus elevating SphK activity. Enhanced SphK activity was also reflected on a functional level, as detected by a commercially available ATP depletion assay used for measuring the enzymatic activity of SphK. Infection of hCMEC/D3 cells with pilus-deficient mutants resulted in a lower SphK activation compared to the N. meningitidis wild type strain. hCMEC/D3 treatment with pilus-enriched protein fractions showed SphK activation similar to the infection with living bacteria and could be ascribed to pilus interaction with the membrane-proximal domain of cellular surface receptor CD147. Inhibition of SphK1 or SphK2 through pre-treatment with specific inhibitors or RNA interference reduced uptake of N. meningitidis into hCMEC/D3 cells, as measured with Gentamicin protection assays. Released S1P induced the phospho-activation of epidermal growth factor receptor (EGFR) via S1PR2 activation, whose expression was also increasing during infection. Furthermore, S1PR2 blockage had a preventive effect on bacterial invasion into hCMEC/D3 cells. On the contrary, activation of S1PR1+3 also reduced bacterial uptake, indicating an opposing regulatory role of S1PR1+3 and S1PR2 during N. meningitidis uptake. Moreover, SphK2 inhibition prevented inflammatory cytokine expression as well as release of interleukin-8 after N. meningitidis infection. Taken together, this study demonstrates the central role of S1P and its cognate receptors S1PR1-3 in the pathophysiology of meningococcal meningitis.
