Purpose
To compare the performance of the dominant and nondominant hand during fundamental arthroscopic simulator training.
Methods
Surgical trainees who participated in a 2-day simulator training course between 2021 and 2023 were classified, according to their arthroscopic experience in beginners and competents. Only right-handed individuals with complete data sets were included in the study. Ambidexterity was trained using a box trainer (Fundamentals of Arthroscopic Surgery Training, Virtamed AG, Schlieren, Switzerland).Two tasks, periscoping for learning camera guidance and triangulation for additional instrument handling, were performed 4 times with the camera in the dominant hand and then in the nondominant hand. For each task, exercise time, camera path length, and instrument path length were recorded and analyzed.
Results
Out of 94 participants 74 right-handed individuals (22 females, 52 males) were classified to novices (n = 43, less than 10 independently performed arthroscopies) and competents (n = 31, more than 10 independently performed arthroscopies). Competents performed significantly better than novices. No significant difference was found after changing the guiding hand for the camera from the dominant to the nondominant hand regarding the camera path length and the instrument path length. Notably, tasks were performed even faster when using the camera in the nondominant hand.
Conclusions
Our data demonstrate that the learned manual skills during basic arthroscopic training are quickly transferred to the contralateral side. In consequence, additional fundamental skills training for camera guidance and instrument handling of the nondominant hand are not necessary.
Clinical Relevance
For skillful arthroscopy, camera guidance and instrument handing must be equally mastered with both hands. It is important to understand how hand dominance may affect learning during arthroscopic simulator training.
In recent years, the applications and accessibility of Virtual Reality (VR) for the healthcare sector have continued to grow. However, so far, most VR applications are only relevant in research settings. Information about what healthcare professionals would need to independently integrate VR applications into their daily working routines is missing. The actual needs and concerns of the people who work in the healthcare sector are often disregarded in the development of VR applications, even though they are the ones who are supposed to use them in practice. By means of this study, we systematically involve health professionals in the development process of VR applications. In particular, we conducted an online survey with 102 healthcare professionals based on a video prototype which demonstrates a software platform that allows them to create and utilise VR experiences on their own. For this study, we adapted and extended the Technology Acceptance Model (TAM). The survey focused on the perceived usefulness and the ease of use of such a platform, as well as the attitude and ethical concerns the users might have. The results show a generally positive attitude toward such a software platform. The users can imagine various use cases in different health domains. However, the perceived usefulness is tied to the actual ease of use of the platform and sufficient support for learning and working with the platform. In the discussion, we explain how these results can be generalized to facilitate the integration of VR in healthcare practice.
Breast cancer (BC) patients often ask for a healthy diet. Here, we investigated a healthy standard diet (SD), a low carb diet (LCD), and a ketogenic diet (KD) for BC patients during the rehabilitation phase. KOLIBRI was an open-label non-randomized one-site nutritional intervention trial, combining inpatient and outpatient phases for 20 weeks. Female BC patients (n = 152; mean age 51.7 years) could select their diet. Data collected were: Quality of life (QoL), spiroergometry, body composition, and blood parameters. In total 30, 92, and 30 patients started the KD, LCD, and SD, respectively. Of those, 20, 76, and 25 completed the final examination. Patients rated all diets as feasible in daily life. All groups enhanced QoL, body composition, and physical performance. LCD participants showed the most impressive improvement in QoL aspects. KD participants finished with a very good physical performance and muscle/fat ratio. Despite increased cholesterol levels, KD patients had the best triglyceride/high-density lipoprotein (HDL) ratio and homeostatic model assessment of insulin resistance index (HOMA-IR). Most metabolic parameters significantly improved in the LCD group. SD participants ended with remarkably low cholesterol levels but did not improve triglyceride/HDL or HOMA-IR. In conclusion, both well-defined KDs and LCDs are safe and beneficial for BC patients and can be recommended during the rehabilitation phase.
Background: Multimodal rehabilitation improves fatigue and mobility in persons with multiple sclerosis (PwMS). Effects are transient and may be conserved by internet-based physical activity promotion programs. Objective: Evaluate the effects of internet-based physical activity and exercise promotion on fatigue, quality of life, and gait in PwMS after inpatient rehabilitation. Methods: PwMS (Expanded Disability Status Scale (EDSS) ≤ 6.0, fatigue: Würzburg Fatigue Inventory for Multiple Sclerosis (WEIMuS) ≥ 32) were randomized into an intervention group (IG) or a control group (CG). After rehabilitation, IG received 3 months of internet-based physical activity promotion, while CG received no intervention. Primary outcome: self-reported fatigue (WEIMuS). Secondary outcomes: quality of life (Multiple Sclerosis Impact Scale 29, MSIS-29), gait (2min/10m walking test, Tinetti score). Measurements: beginning (T0) and end (T1) of inpatient rehabilitation, 3 (T2) and 6 (T3) months afterwards. Results: 64 of 84 PwMS were analyzed (IG: 34, CG: 30). After rehabilitation, fatigue decreased in both groups. At T2 and T3, fatigue increased again in CG but was improved in IG (p < 0.001). MSIS-29 improved in both groups at T1 but remained improved at T2 and T3 only in IG. Gait improvements were more pronounced in IG at T2. Conclusions: The study provides Class II evidence that the effects of rehabilitation on fatigue, quality of life, and gait can be maintained for 3–6 months with an internet-based physical activity and exercise promotion program.
Die Rehabilitation von Schlaganfallpatienten erfordert ein interdisziplinäres Vorgehen. Dies ist im klinischen Alltag oft nur schwer umsetzbar. Im Jahr 2011 wurde daher im Neurologischen Rehabilitationszentrum Quellenhof in Bad Wildbad ein spezielles Behandlungskonzept für Schlaganfallpatienten entwickelt. Mit dieser Studie sollte die Wirksamkeit dieses neuen Konzepts untersucht werden. Dabei wurde die Behandlung im Schlaganfallkonzept mit der bisher üblichen Behandlung verglichen. Zielparameter waren der Barthel-Index und die modifizierte Rankin-Skala bei Aufnahme und bei Entlassung. Die Ergebnisse zeigen, dass sowohl die bisherige Behandlung als auch die Behandlung im Schlaganfallkonzept effektiv sind. Im Schlaganfallkonzept konnte jedoch ein größerer Zugewinn an alltagsrelevanten Fähigkeiten erzielt werden; zudem berichteten Pflegekräfte und Therapeuten eine verbesserte Zusammenarbeit. Somit steigert das Schlaganfallkonzept nicht nur die Alltagskompetenz der Patienten, sondern auch den Wissenstransfer zwischen den Berufsgruppen und die Interdisziplinarität.
Background: The rehabilitation of gait disorders in patients with multiple sclerosis (MS) and stroke is often based on conventional treadmill training. Virtual reality (VR)-based treadmill training can increase motivation and improve therapy outcomes. The present study evaluated an immersive virtual reality application (using a head-mounted display, HMD) for gait rehabilitation with patients to (1) demonstrate its feasibility and acceptance and to (2) compare its short-term effects to a semi-immersive presentation (using a monitor) and a conventional treadmill training without VR to assess the usability of both systems and estimate the effects on walking speed and motivation. Methods: In a within-subjects study design, 36 healthy participants and 14 persons with MS or stroke participated in each of the three experimental conditions (VR via HMD, VR via monitor, treadmill training without VR). Results: For both groups, the walking speed in the HMD condition was higher than in treadmill training without VR and in the monitor condition. Healthy participants reported a higher motivation after the HMD condition as compared with the other conditions. Importantly, no side effects in the sense of simulator sickness occurred and usability ratings were high. No increases in heart rate were observed following the VR conditions. Presence ratings were higher for the HMD condition compared with the monitor condition for both user groups. Most of the healthy study participants (89%) and patients (71%) preferred the HMD-based training among the three conditions and most patients could imagine using it more frequently. Conclusions For the first time, the present study evaluated the usability of an immersive VR system for gait rehabilitation in a direct comparison with a semi-immersive system and a conventional training without VR with healthy participants and patients. The study demonstrated the feasibility of combining a treadmill training with immersive VR. Due to its high usability and low side effects, it might be particularly suited for patients to improve training motivation and training outcome e. g. the walking speed compared with treadmill training using no or only semi-immersive VR. Immersive VR systems still require specific technical setup procedures. This should be taken into account for specific clinical use-cases during a cost-benefit assessment.
The minimal clinically important difference (MCID) defines to what extent change on a health status instrument is clinically relevant, which aids scientists and physicians in measuring therapy effects. This is the first study that aimed to establish the MCID of the Clinical chronic obstructive pulmonary disease (COPD) Questionnaire (CCQ), the COPD Assessment Test (CAT) and the St George’s Respiratory Questionnaire (SGRQ) in the same pulmonary rehabilitation population using multiple approaches. In total, 451 COPD patients participated in a 3-week Pulmonary Rehabilitation (PR) programme (58 years, 65% male, 43 pack-years, GOLD stage II/III/IV 50/39/11%). Techniques used to assess the MCID were anchor-based approaches, including patient-referencing, criterion-referencing and questionnaire-referencing, and the distribution-based methods standard error of measurement (SEM), 1.96SEM and half standard deviation (0.5s.d.). Patient- and criterion-referencing led to MCID estimates of 0.56 and 0.62 (CCQ); 3.12 and 2.96 (CAT); and 8.40 and 9.28 (SGRQ). Questionnaire-referencing suggested MCID ranges of 0.28–0.61 (CCQ), 1.46–3.08 (CAT) and 6.86–9.47 (SGRQ). The SEM, 1.96SEM and 0.5s.d. were 0.29, 0.56 and 0.46 (CCQ); 3.28, 6.43 and 2.80 (CAT); 5.20, 10.19 and 6.06 (SGRQ). Pooled estimates were 0.52 (CCQ), 3.29 (CAT) and 7.91 (SGRQ) for improvement. MCID estimates differed depending on the method used. Pooled estimates suggest clinically relevant improvements needing to exceed 0.40 on the CCQ, 3.00 on the CAT and 7.00 on the SGRQ for moderate to very severe COPD patients. The MCIDs of the CAT and SGRQ in the literature might be too low, leading to overestimation of treatment effects for patients with COPD.
Background
Fibromyalgia syndrome (FMS) is a complex chronic condition that makes high demands on patients’ self-management skills. Thus, patient education is considered an important component of multimodal therapy, although evidence regarding its effectiveness is scarce. The main objective of this study is to assess the effectiveness of an advanced self-management patient education program for patients with FMS as compared to usual care in the context of inpatient rehabilitation.
Methods/Design
We conducted a multicenter cluster randomized controlled trial in 3 rehabilitation clinics. Clusters are groups of patients with FMS consecutively recruited within one week after admission. Patients of the intervention group receive the advanced multidisciplinary self-management patient education program (considering new knowledge on FMS, with a focus on transfer into everyday life), whereas patients in the control group receive standard patient education programs including information on FMS and coping with pain. A total of 566 patients are assessed at admission, at discharge and after 6 and 12 months, using patient reported questionnaires. Primary outcomes are patients’ disease- and treatment-specific knowledge at discharge and self-management skills after 6 months. Secondary outcomes include satisfaction, attitudes and coping competences, health-promoting behavior, psychological distress, health impairment and participation. Treatment effects between groups are evaluated using multilevel regression analysis adjusting for baseline values.
Discussion
The study evaluates the effectiveness of a self-management patient education program for patients with FMS in the context of inpatient rehabilitation in a cluster randomized trial. Study results will show whether self-management patient education is beneficial for this group of patients.
The Quality of Acute Stroke Care-an Analysis of Evidence-Based Indicators in 260 000 Patients

Background: Stroke patients should be cared for in accordance with evidence-based guidelines. The extent of implementation of guidelines for the acute care of stroke patients in Germany has been unclear to date. Methods: The regional quality assurance projects that cooperate in the framework of the German Stroke Registers Study Group (Arbeitsgemeinschaft Deutscher Schlaganfall-Register, ADSR) collected data on the care of stroke patients in 627 hospitals in 2012. The quality of the acute hospital care of patients with stroke or transient ischemic attack (TIA) was assessed on the basis of 15 standardized, evidence-based quality indicators and compared across the nine participating regional quality assurance projects. Results: Data were obtained on more than 260 000 patients nationwide. Intravenous thrombolysis was performed in 59.7% of eligible ischemic stroke patients patients (range among participating projects, 49.7-63.6%). Dysphagia screening was documented in 86.2% (range, 74.8-93.1%). For the following indicators, the defined targets were not reached for all of Germany: antiaggregation within 48 hours, 93.4% (range, 86.6-96.4%); anticoagulation for atrial fibrillation, 77.6% (range, 72.4-80.1%); standardized dysphagia screening, 86.2% (range, 74.8-93.1%); oral and written information of the patients or their relatives, 86.1% (range, 75.4-91.5%). The rate of patients examined or treated by a speech therapist was in the target range. Conclusion: The defined targets were reached for most of the quality indicators. Some indicators, however, varied widely across regional quality assurance projects. This implies that the standardization of care for stroke patients in Germany has not yet been fully achieved.
Der freie an der Arteria radialis gestielte fasziokutane Unterarmlappen hat sich als Standardmethode für die Rekonstruktion nach Resektion ausgedehnter Tumoren im Kopf-Hals-Bereich etabliert. Hierzu trug nicht nur die überschaubare und komplikationsarme Operationstechnik bei, sondern auch die Möglichkeit der Wiederherstellung von durch den Tumor und dessen Resektion in Mitleidenschaft gezogenen Funktionen und somit die frühzeitige Rehabilitation und die Verbesserung der Lebensqualität der Patienten. In dieser Studie wurden Operationsfolgen und Komplikationen, funktionelle Einschränkungen und die postoperative Lebensqualität der Patienten für das an der Würzburger HNO-Universitätsklinik operierte Patientengut dargestellt. Neben dem Aktenstudium wurden hierfür Patientenfragebögen ausgewertet und die Patienten wurden nachuntersucht. Es konnten viele durch die operative Therapie und die adjuvante Strahlen- und Chemotherapie ausgelöste funktionelle Störungen festgestellt werden. Im Vordergrund standen dabei Schluckbeschwerden und Probleme mit dem Sprechen bzw. der Stimme. Eingeschränkte Beweglichkeit im Schulterbereich und Lymphödeme am Hals als typische Folgen der Neck Dissection kamen ebenfalls sehr häufig vor. Oft wurden auch Mucositis und Mundtrockenheit als Folgen der Strahlen- und Chemotherapie beobachtet. Auffällig war, dass die Häufigkeit der festgestellten Operationsfolgen davon abhängig war, ob sie vom untersuchenden Arzt oder vom Patienten selbst beobachtet wurden, was die Wichtigkeit von Fragebögen zur postoperativen Beurteilung der Operationsfolgen deutlich macht. Durch die Fragebögen konnte darüber hinaus die Lebensqualität der Patienten beurteilt werden. Die Auswertung ergab in den meisten Bereichen ein mittleres bis hohes Maß an Zufriedenheit mit den abgefragten Aspekten der Lebensqualität. Nur wenige Patienten gaben einzelne schlechte Bewertungen, vor allem die funktionellen Beschwerden wurden negativ bewertet. Trotzdem waren fast alle Patienten mit der Therapie zufrieden und würden sich wieder genauso entscheiden. Insgesamt konnte also gezeigt werden, dass trotz der häufig festgestellten funktionellen Einschränkungen ein hohes Maß an subjektiver Lebensqualität erreicht werden kann und eine Anpassung an die momentane Lebenssituation stattfindet.
Das Auftreten neurologischer Defizite bei Patienten mit der Diagnose Koronare Herzkrankheit (KHK) ist bekannt. Als Ursachen werden lange Ischämiezeiten und Mikroluftembolien (Bubbles) während einer Aorto-Koronaren-Venenbypass- Operation (ACVB-Operation) angenommen Patienten, die der kardiologischen Intervention mittels Perkutaner Transluminaler Koronarer Angioplastie (PTCA / PCI) zugeführt werden, zeigen ebenfalls neurologische Defizite. Bisher waren neurokognitive Veränderungen in der kardiologischen Rehabilitation nur Gegenstand sehr weniger Studien. Über den zeitlichen Verlauf während der Anschlussheilbehandlung (AHB) wurden noch keine Untersuchungen durchgeführt. Aus diesem Grund beschäftigt sich diese Arbeit mit der Testung des neurologischen Status der Patienten mit einer KHK bei Aufnahme und Entlassung aus der Rehabilitation. Es sollte geklärt werden, ob und in welchem Ausmaß kognitive Defizite bestehen und wie sich diese über den Zeitraum der Rehabilitation verändern. Dazu sieht die Studie einen Vergleich zwischen zwei Kohorten vor. Die erste Gruppe setzt sich aus Patienten zusammen, die mittels PTCA versorgt wurden. Die zweite Gruppe schließt einer ACVB-Operation zugeführte Herzpatienten ein. Dass KHK-Patienten neurokognitive Defizite aufweisen hat sich in den Studienergebnissen bestätigt. Sowohl die PCI-Kohorte, als auch die ACVB-Gruppe zeigte bei Aufnahme neurokognitive Leistungseinschränkungen in Form unterdurchschnittlicher Testergebnisse. Die Hypothese, dass im Rahmen einer AHB eine Verminderung neurokognitiver Leistungseinschränkungen und bessere Testresultate bei Entlassung erzielt werden, wurde widerlegt. Die Annahme, dass PCI-Patienten geringere kognitive Defizite und bessere Testleistungen bei Aufnahme und Entlassung verzeichnen als ACVB-Patienten trifft nicht zu. Die Vermutung, dass ACVB-Patienten ausgeprägtere neurologische Leistungseinschränkungen und schlechtere Testresultate bei Aufnahme und Entlassung aufzeigen, hat sich nicht bewahrheitet.
Die Critical-lllness-Polyneuropathie (CIP), eine Erkrankung des peripheren Nervensystems nach einer schweren intensivmedizinisch behandlungspflichtigen Erkrankung, ist bereits seit dem 19. Jahrhundert bekannt und lässt sich nur sehr schwer von einer Critical-Illness- Myopathie, die im gleichen Kontext auftreten kann, unterscheiden. Erschwert wird die Situation dadurch, dass beide Funktionsstörungen kombiniert auftreten können. Auf Grund der Weiterentwicklung in der Medizin ist in den letzten 2 Jahrzehnten eine Häufigkeitssteigerung zu verzeichnen mit der Forderung geeignete Maßnahmen zu finden, die häufig schwerwiegenden Folgeerscheinungen zu mindern. Mit Entwicklung eines bestimmten neurorehabilitativen Behandlungsregimes ergeben sich die Fragen, welche CIP Patienten profitieren, ergeben sich prognoserelevante Faktoren und welche Art und Dauer der Neurorehabilitation vorgehalten werden muss. In der vorliegenden Arbeit wurden retrospektiv die Entlassungsbriefe der Patienten mit einer CIP der Abteilung für neurologisch-neurochirurgisch Frührehabilitation der Jahre 2004-2008 ausgewertet. Die Beurteilung der Fähigkeitsstörungen erfolgte mit dem Frührehabilitations-Barthel-Index nach Schönle, dem Barthel-Index sowie dem FIM und das Handicap wurde mit der 8-stufigen Glasgow-Outcome-Scale bewertet. Zusätzliche Parameter waren das Alter und Geschlecht, die Ursache der CIP, die Zeit von Erkrankungsbeginn bis zur Aufnahme, die Behandlungsdauer, Komplikationen sowie die Entlassungsart. 200 Patienten, 67,5% Männer und 32,5% Frauen konnten ausgewertet werden. Eine Häufung des Erkrankungsbildes fand sich bei Patienten ab dem 60. Lebensjahr. Eine Ursachenpräferenz fand sich nicht, wobei im Wesentlichen kardiale, respiratorische und gastrointestinale Erkrankungen vorgefunden worden waren. Alle Patienten konnten, nach einer durchschnittlichen Behandlungsdauer von 40 Tagen, von der Frührehabilitationsbehandlung profitieren. So konnten 34% in weiterführende rehabilitative Behandlungsstufen und 17,5% der Patienten gebessert nach Hause entlassen werden. Die Sterblichkeit mit 11% muss der besonderen Schwere des Krankheitsbildes angelastet werden. Dies wird durch die Feststellung gestützt, dass die Komplikationsrate mit über 60% während der rehabilitativen Behandlung sehr hoch lag. Hervorzuheben ist, dass sich herausarbeiten ließ, dass sich eine längere Behandlung positiv auf die Behandlungsergebnisse, auch bei schwer Betroffenen auswirkte. Die auslösende Ursache scheint keinen wesentlichen Einfluss auf das outcome zu haben. Männer profitieren bei der Wiedererlangung motorischer Fähigkeiten etwas besser als Frauen. Die älteren Patienten zeigen gute Verbesserungen bei körperlichen Funktionsstörungen. Bezogen auf die kognitiven Fähigkeiten muss aber das Alter als negativer Prädiktor angesehen werden. Der Grad der Behinderung hat einen deutlichen Einfluss auf die Behandlungsdauer und Behandlungsergebnisse. Demgegenüber stellt eine primäre Intensivpflicht einen negativen Prädiktor, auch wenn auch diese Patienten von der Rehabilitation profitieren.
Die vorliegende Arbeit überprüft an einem nach Alter, Geschlecht, Barthel-Index und Mini-Mental-State-Test gematchten geriatrischen Patientenkollektiv mit erstmaligem Schlaganfall die Wirksamkeit einer vorausgegangenen Akutbehandlung an einer Stroke Unit (n=59) gegenüber einer allgemeinen (internistischen oder neurologischen) stationären Akutbehandlung (n=59) für die Prognose im Laufe einer nachfolgenden geriatrischen Rehabilitationsbehandlung. Hintergrund dieser Frage ist der erhöhte ökonomische Druck im Gesundheitswesen, der eine Effizienzprüfung einer personell, technisch und logistisch aufwändigeren und damit teureren Behandlung auf einer Spezialstation verlangt. Bei Anwendung zahlreicher funktioneller Skalen und Erhebung einiger sozioökonomischer Faktoren zeigte sich auf Signifikanzniveau, dass die auf Stroke Unit Vorbehandelten bei Aufnahme in die Rehabilitation motorisch schwerer beeinträchtigt waren (timed up and go-Test p=0,044, Lachs-Test p=0,34) und sich dann ausgeprägter (Transferleistung p=0,024) auf ein bei Rehabilitationsende schließlich vergleichbares Leistungsniveau verbesserten. Die ursprünglich geplante Langzeiteffizienzbetrachtung im Gruppenvergleich scheiterte an Datenschutzbedenken. Gesundheitsökonomisch relevant ist, dass die Vorverweildauer im Akutkrankenhaus bei Stroke Unit-Patienten sechs Tage kürzer war, die Rehabilitationsdauer allerdings vier Tage länger. Weitergehende Kostenbetrachtungen scheiterten am Unwillen zur Leistungsoffenlegung verschiedener Beteiligter im Gesundheitssystem. Eine plausible Erklärung für diese positive motorische Leistungsweiterentwicklung nach Stroke Unit-Vorbehandlung kann in einer frühzeitigeren und effektiveren Anstrengung durch Krankengymnastik, Ergotherapie, Logopädie, aktivierende Pflege, „enriched environment“ gesucht werden, die sich positiv auf die Plastizität im Gehirn als wesentliche Bedingung zur Funktionswiedergewinnung auswirken könnte, was aber noch umstritten ist und Ziel weiterer Untersuchungen sein muss.
Auf Grund der demografischen Entwicklung nimmt die Bedeutung der Altersmedizin und mit ihr diejenige der geriatrischen Rehabilitation zu. Durch gesetzliche Vorgaben und klinische Überlegungen soll die teilstationäre Behandlung in Zukunft einen größeren Stellenwert bekommen. Vor dem Hintergrund des zunehmenden Kostendrucks werden in dieser Studie prognostische Faktoren für den funktionellen Rehabilitationserfolg (Pflegebedürftigkeit) und die Aufenthaltsdauer bei geriatrischen Tagesklinikpatienten mit den Diagnosen Apoplex und Fraktur untersucht. Mit der multiplen Regressionsanalyse konnte ein Vorhersagewert für den Barthel-Index bei Entlassung für die Diagnosegruppe Apoplex durch den Barthel-Index bei Aufnahme und das geriatrische Screening nach Lachs ermittelt werden; außerdem für die Diagnosegruppe Fraktur durch den Barthel-Index bei Aufnahme und den Motilitätstest nach Tinetti.
