Natrium-Glukose Transporter (SGLT) gehören zur „solute carrier 5“ (SLC5) Familie, die sich durch einen sekundär aktiven, natriumabhängigen Transport von Zuckern und an-deren Molekülen nach intrazellulär auszeichnen. Die durch das Gen SLC5A4 kodierte Isoform SGLT3 transportiert dagegen keinen Zucker, sondern verhält sich als Glukosesensor, der nach Bindung seiner Liganden eine Membrandepolarisation induziert. In genomweiten Exomsequenzierungsstudien (whole exome sequencing, WES) mehrerer erweiterter Stammbäume mit hoher Prävalenz des Aufmerksamkeitsdefizit-/Hyperaktivitätssyndroms (ADHS) wurde im Vorfeld eine ATG-Tripletdeletion in SLC5A4 identifiziert, die zum Verlust einer Aminosäure (ΔM500) in SGLT3 führt und zumindest partiell mit dem klinischen Phänotyp kosegregiert.
In der vorliegenden Arbeit wurde die zentralnervöse Expression von SGLT3 auf RNA- Ebene mittels Reverse-Transkriptase PCR sowie real-time PCR aus humanen Gesamt-RNAs nachgewiesen. Dabei konnte eine ubiquitäre Expression im Gehirn mit relativ erhöhter Expression unter anderem in Striatum und Hypothalamus, deren Dysfunktion in der Pathogenese des ADHS impliziert wurde, gezeigt werden. Da Mutationen in homologen Domänen der eng strukturverwandten Isoformen SGLT1 und SGLT2 sowohl intestinale als auch renale Funktionen schwer beeinträchtigen, wurden in dieser Arbeit funktionelle Charakteristika sowohl des wildtypischen als auch der ΔM500 und der benachbarten ΔI501 Deletionsvariante von SGLT3 mittels Zwei-Elektroden Spannungs- und Stromklemme in entsprechend cRNA-injizierten Xenopus laevis Oozyten untersucht. Der hochpotente SGLT3-spezifische Iminozuckeragonist 1-Desoxynojirimycin (DNJ) induzierte an SGLT3-exprimierenden Oozyten in sauren Bedingungen etwa dreifach größere Kationeneinströme als D-Glukose, was sowohl im Spannungsklemmen-, und anhand einer entsprechenden Membrandepolarisation im Stromklemmenmodus gezeigt wurde. Die mit der ΔM500 bzw. ΔI501 Variante injizierten Oozyten dagegen zeigten in den maximalen Aktivierungsbedingungen um 92% bzw. 96% (p<0,01) reduzierte Kationeneinströme, sodass diese als hochgradig schädliche „Loss of Function“ Mutationen in SGLT3 charakterisiert wurden. Dieser Befund wurde mittels bioinformatischer in-silico Effektvorhersage validiert.
Um Konsequenzen der Sequenzalteration auf den Membraneinbau der Transporter zu untersuchen, wurden die mit einem gelb fluoreszierenden Farbstoff (YFP) markierten Transporter in Oozytenmembranen mittels Laser-Scanning Mikroskop nachgewiesen und die jeweiligen Mengen der Konstrukte anhand der Fluoreszenzintensitäten quantifiziert. Dabei zeigte sich eine um 53% bzw. 42% (p<0,01) reduzierte Menge der mutierten Konstrukte ΔM500 bzw. ΔI501 in der Membran, was zusätzliche schädliche Effekte der Mutationen auf das sogenannte Membrantargeting der Transporter belegt.
Zusammenfassend demonstrieren die Ergebnisse dieser Arbeit, dass die ΔM500 Variante von SGLT3, welcher in ADHS-relevanten Hirnarealen exprimiert wird, dessen sub-stratinduzierte Natriumleitfähigkeit aufhebt und den Membraneinbau beeinträchtigen könnte, was in Wechselwirkung mit anderen genetischen ADHS Risikovarianten das Risiko für ADHS in Mutationsträgern beeinflussen kann.
Ziel der Doktorarbeit war die Implementierung eines Herzinfarktnetzes im ländlichen Raum mit Hilfe der FITT-STEMI-Studie und damit die Optimierung der Abläufe der Behandlungen und Verbesserung der Prognose der Patienten. Die Ergebnisse wurden quartalsweise mit den beteiligten Berufsgruppen wie Notärzten, Sanitätern, Personal der Intensivstation, den einweisenden Kliniken etc. besprochen und so ein Netzwerk aufgebaut. Dadurch konnte das Einzugsgebiet erweitert werden, die Primärtransporte nahmen von 54 auf 65 % und die telefonischen Infarktankündigungen von 43 auf 80 % zu, zudem konnte die door-to-catheter-Zeit bei Primärtransporten von 40 auf 23 min. gesenkt werden.
Das bekapselte, Gram-negative, diplokokkenförmige Bakterium Neisseria meningitidis (Nme) ist ein asymptomatischer Kommensale des oberen Nasenrachenraums im Men-schen. Gerade bei Kindern ist es dem humanspezifischen Pathogen in seltenen Fällen möglich, in den Blutstrom einzuwandern und lebensbedrohliche Krankheitsbilder wie Meningoenzephalitis und Sepsis auszulösen, welche als „Invasive Meningokokkener-krankung“ (IMD) zusammengefasst werden. Jährlich ereignen sich weltweit bis zu 1,2 Mio Fälle von IMD, welche aufgrund des fulminanten Verlaufs und der hohen Letalität gefürchtet sind. In der Bekämpfung der Nme-Sepsis ist das humane Komplementsystem von entscheidender Bedeutung. Vor diesem Hintergrund ist die protektive Rolle des lytischen (Membranangriffskomplex MAK) und opsonisierenden Arms (Opsonine iC3b und C1q) der Komplementkaskade gut dokumentiert. Dagegen ist der Beitrag des in-flammatorischen Arms (Anaphylatoxine C3a und C5a) in der Nme-Sepsis bisher unklar. Aus diesem Grunde wurde mit dieser Arbeit die Rolle des inflammatorischen Arms an-hand des Komplement C5a-Rezeptors 1 (C5aR1) in der Pathophysiologie der Nme-Sepsis am Mausmodell untersucht. Nach Etablierung des murinen, intraperitonealen In-fektionsmodells konnte ein schädlicher Effekt des C5aR1 in der Nme-Sepsis beobachtet werden. Aus der Abwesenheit des C5aR1 resultierte eine höhere Überlebensrate, ein besserer klinischer Zustand, eine niedrigere Bakteriämie und niedrigere Konzentrationen der pro-inflammatorischen Mediatoren IL-6, CXCL-1 und TNF-α. Im Hinblick auf den zellulären Pathomechanismus sprechen Ergebnisse dieser Arbeit dafür, dass der C5aR1 primär eine gesteigerte Freisetzung inflammatorischer Mediatoren durch verschiedene Zellpopulationen triggert (Zytokinsturm), wodurch sekundär Zellparalyse, steigende Bakteriämie und höhere Letalität bedingt sind. Durch Depletionsversuche und Immun-fluoreszenzfärbungen konnte, unabhängig vom C5aR1, eine allgemein protektive Rolle von neutrophilen Granulozyten und Monozyten/Makrophagen in der Nme-Sepsis beo-bachtet werden. Darüber hinaus präsentierte sich der zyklische C5aR1-Antagonist PMX205 als erfolgsversprechende Therapieoption, um Parameter einer murinen Nme-Sepsis zu verbessern. Weitere Untersuchungen sind nötig, um die Wirksamkeit dieser Substanz in der humanen Nme-Sepsis zu erforschen. Zudem könnte das murine, intrape-ritoneale Infektionsmodell zur Klärung der Rolle des C5aR2 in der Nme-Sepsis genutzt werden.
Base J, β-D-glucosyl-hydroxymethyluracil, is a chromatin modification of thymine in the nuclear DNA of flagellated protozoa of the order Kinetoplastida. In Trypanosoma brucei, J is enriched, along with histone H3 variant (H3.V), at sites involved in RNA Polymerase (RNAP) II termination and telomeric sites involved in regulating variant surface glycoprotein gene (VSG) transcription by RNAP I. Reduction of J in T. brucei indicated a role of J in the regulation of RNAP II termination, where the loss of J at specific sites within polycistronic gene clusters led to read-through transcription and increased expression of downstream genes. We now demonstrate that the loss of H3.V leads to similar defects in RNAP II termination within gene clusters and increased expression of downstream genes. Gene derepression is intensified upon the subsequent loss of J in the H3.V knockout. mRNA-seq indicates gene derepression includes VSG genes within the silent RNAP I transcribed telomeric gene clusters, suggesting an important role for H3.V in telomeric gene repression and antigenic variation. Furthermore, the loss of H3.V at regions of overlapping transcription at the end of convergent gene clusters leads to increased nascent RNA and siRNA production. Our results suggest base J and H3.V can act independently as well as synergistically to regulate transcription termination and expression of coding and non-coding RNAs in T. brucei, depending on chromatin context (and transcribing polymerase). As such these studies provide the first direct evidence for histone H3.V negatively influencing transcription elongation to promote termination.
We identified the dsRNA binding protein RbdB as an essential component in miRNA processing in Dictyostelium discoideum. RbdB is a nuclear protein that accumulates, together with Dicer B, in nucleolar foci reminiscent of plant dicing bodies. Disruption of rbdB results in loss of miRNAs and accumulation of primary miRNAs. The phenotype can be rescued by ectopic expression of RbdB thus allowing for a detailed analysis of domain function. The lack of cytoplasmic dsRBD proteins involved in miRNA processing, suggests that both processing steps take place in the nucleus thus resembling the plant pathway. However, we also find features e.g. in the domain structure of Dicer which suggest similarities to animals. Reduction of miRNAs in the rbdB- strain and their increase in the Argonaute A knock out allowed the definition of new miRNAs one of which appears to belong to a new non-canonical class.
Background
Fabry disease (FD) is an X-linked multisystemic disorder with a heterogeneous phenotype. Especially atypical or late-onset type 2 phenotypes present a therapeutical dilemma.
Methods
To determine the clinical impact of the alpha-Galactosidase A (GLA) p.A143T/ c.427G > A variation, we retrospectively analyzed 25 p.A143T patients in comparison to 58 FD patients with other missense mutations.
Results
p.A143T patients suffering from stroke/ transient ischemic attacks had slightly decreased residual GLA activities, and/or increased lyso-Gb3 levels, suspecting FD. However, most male p.A143T patients presented with significant residual GLA activity (~50 % of reference), which was associated with normal lyso-Gb3 levels. Additionally, p.A143T patients showed less severe FD-typical symptoms and absent FD-typical renal and cardiac involvement in comparison to FD patients with other missense mutations. Two tested female p.A143T patients with stroke/TIA did not show skewed X chromosome inactivation. No accumulation of neurologic events in family members of p.A143T patients with stroke/transient ischemic attacks was observed.
Conclusions
We conclude that GLA p.A143T seems to be most likely a neutral variant or a possible modifier instead of a disease-causing mutation. Therefore, we suggest that p.A143T patients with stroke/transient ischemic attacks of unknown etiology should be further evaluated, since the diagnosis of FD is not probable and subsequent ERT or chaperone treatment should not be an unreflected option.
Nosological delineation of congenital ocular motor apraxia type Cogan: an observational study

Background
The nosological assignment of congenital ocular motor apraxia type Cogan (COMA) is still controversial. While regarded as a distinct entity by some authorities including the Online Mendelian Inheritance in Man catalog of genetic disorders, others consider COMA merely a clinical symptom.
Methods
We performed a retrospective multicenter data collection study with re-evaluation of clinical and neuroimaging data of 21 previously unreported patients (8 female, 13 male, ages ranging from 2 to 24 years) diagnosed as having COMA.
Results
Ocular motor apraxia (OMA) was recognized during the first year of life and confined to horizontal pursuit in all patients. OMA attenuated over the years in most cases, regressed completely in two siblings, and persisted unimproved in one individual. Accompanying clinical features included early onset ataxia in most patients and cognitive impairment with learning disability (n = 6) or intellectual disability (n = 4). Re-evaluation of MRI data sets revealed a hitherto unrecognized molar tooth sign diagnostic for Joubert syndrome in 11 patients, neuroimaging features of Poretti-Boltshauser syndrome in one case and cerebral malformation suspicious of a tubulinopathy in another subject. In the remainder, MRI showed vermian hypo-/dysplasia in 4 and no abnormalities in another 4 patients. There was a strong trend to more severe cognitive impairment in patients with Joubert syndrome compared to those with inconclusive MRI, but otherwise no significant difference in clinical phenotypes between these two groups.
Conclusions
Systematical renewed analysis of neuroimaging data resulted in a diagnostic reappraisal in the majority of patients with early-onset OMA in the cohort reported here. This finding poses a further challenge to the notion of COMA constituting a separate entity and underlines the need for an expert assessment of neuroimaging in children with COMA, especially if they show cognitive impairment.
Long non-coding RNAs are important regulators of gene expression and signaling pathways. The expression of long ncRNAs is dysregulated in cancer and other diseases. The identification and characterization of long ncRNAs is often challenging due to their low expression level and localization to chromatin. Here, we identify a functional long ncRNA, PARROT (Proliferation Associated RNA and Regulator Of Translation) transcribed by RNA polymerase II and expressed at a relatively high level in a number of cell lines. The PARROT long ncRNA is associated with proliferation in both transformed and normal cell lines. We characterize the long ncRNA PARROT as an upstream regulator of c-Myc affecting cellular proliferation and translation using RNA sequencing and mass spectrometry following depletion of the long ncRNA. PARROT is repressed during senescence of human mammary epithelial cells and overexpressed in some cancers, suggesting an important association with proliferation through regulation of c-Myc. With this study, we add to the knowledge of cytoplasmic functional long ncRNAs and extent the long ncRNA-Myc regulatory network in transformed and normal cells.
DPF3 (BAF45c) is a member of the BAF chromatin remodeling complex. Two isoforms have been described, namely DPF3a and DPF3b. The latter binds to acetylated and methylated lysine residues of histones. Here, we elaborate on the role of DPF3a and describe a novel pathway of cardiac gene transcription leading to pathological cardiac hypertrophy. Upon hypertrophic stimuli, casein kinase 2 phosphorylates DPF3a at serine 348. This initiates the interaction of DPF3a with the transcriptional repressors HEY, followed by the release of HEY from the DNA. Moreover, BRG1 is bound by DPF3a, and is thus recruited to HEY genomic targets upon interaction of the two components. Consequently, the transcription of downstream targets such as NPPA and GATA4 is initiated and pathological cardiac hypertrophy is established. In human, DPF3a is significantly up-regulated in hypertrophic hearts of patients with hypertrophic cardiomyopathy or aortic stenosis. Taken together, we show that activation of DPF3a upon hypertrophic stimuli switches cardiac fetal gene expression from being silenced by HEY to being activated by BRG1. Thus, we present a novel pathway for pathological cardiac hypertrophy, whose inhibition is a long-term therapeutic goal for the treatment of the course of heart failure.
Out of the myriad of potential DNA binding sites of the glucocorticoid receptor (GR) found in the human genome, only a cell-type specific minority is actually bound, indicating that the presence of a recognition sequence alone is insufficient to specify where GR binds. Cooperative interactions with other transcription factors (TFs) are known to contribute to binding specificity. Here, we reasoned that sequence signals preventing GR recruitment to certain loci provide an alternative means to confer specificity. Motif analyses uncovered candidate Negative Regulatory Sequences (NRSs) that interfere with genomic GR binding. Subsequent functional analyses demonstrated that NRSs indeed prevent GR binding to nearby response elements. We show that NRS activity is conserved across species, found in most tissues and that they also interfere with the genomic binding of other TFs. Interestingly, the effects of NRSs appear not to be a simple consequence of changes in chromatin accessibility. Instead, we find that NRSs interact with proteins found at sub-nuclear structures called paraspeckles and that these proteins might mediate the repressive effects of NRSs. Together, our studies suggest that the joint influence of positive and negative sequence signals partition the genome into regions where GR can bind and those where it cannot.
Novel RNA-guided cellular functions are paralleled by an increasing number of RNA-binding proteins (RBPs). Here we present ‘serial RNA interactome capture’ (serIC), a multiple purification procedure of ultraviolet-crosslinked poly(A)–RNA–protein complexes that enables global RBP detection with high specificity. We apply serIC to the nuclei of proliferating K562 cells to obtain the first human nuclear RNA interactome. The domain composition of the 382 identified nuclear RBPs markedly differs from previous IC experiments, including few factors without known RNA-binding domains that are in good agreement with computationally predicted RNA binding. serIC extends the number of DNA–RNA-binding proteins (DRBPs), and reveals a network of RBPs involved in p53 signalling and double-strand break repair. serIC is an effective tool to couple global RBP capture with additional selection or labelling steps for specific detection of highly purified RBPs.
Bei der Teilung einer Zelle werden das Genom und die Zellbestandteile zwischen zwei Tochterzellen aufgeteilt. Dies erfordert verschiedene fein aufeinander abgestimmte Vorgänge. Unter anderem ist eine proteinreiche Struktur beteiligt, die 1891 entdeckt wurde: der Mittelkörper. In vorliegender Arbeit wurden gezielt gekennzeichnete Mittelkörperproteine analysiert und verschiedene Phasen des Transports unterschieden. Es erfolgten erstmals Messungen unter Nutzung der ZF1-Methode. Zudem wird anhand der ZF1-Technik nachgewiesen, dass im Rahmen der Zellteilung die Trennung der interzellulären Brücke zu beiden Seiten des Mittelkörpers stattfindet, woraufhin dieser nach extrazellulär abgegeben wird und über einen der Phagozytose ähnlichen und von Aktin abhängigen Mechanismus von einer Tochterzelle oder unverwandten Nachbarzelle aufgenommen wird.
Pattern recognition approaches, such as the Support Vector Machine (SVM), have been successfully used to classify groups of individuals based on their patterns of brain activity or structure. However these approaches focus on finding group differences and are not applicable to situations where one is interested in accessing deviations from a specific class or population. In the present work we propose an application of the one-class SVM (OC-SVM) to investigate if patterns of fMRI response to sad facial expressions in depressed patients would be classified as outliers in relation to patterns of healthy control subjects. We defined features based on whole brain voxels and anatomical regions. In both cases we found a significant correlation between the OC-SVM predictions and the patients' Hamilton Rating Scale for Depression (HRSD), i.e. the more depressed the patients were the more of an outlier they were. In addition the OC-SVM split the patient groups into two subgroups whose membership was associated with future response to treatment. When applied to region-based features the OC-SVM classified 52% of patients as outliers. However among the patients classified as outliers 70% did not respond to treatment and among those classified as non-outliers 89% responded to treatment. In addition 89% of the healthy controls were classified as non-outliers.
Introduction: Speckle-tracking echocardiography has recently emerged as a quantitative ultrasound technique for accurately evaluating myocardial function by analyzing the motion of speckles identified. Speckle-tracking obtained under stress may offer an opportunity to improve the detection of dynamic regional abnormalities and myocardial viability.
Objective: To evaluate stress speckle tracking as tool to detect myocardial viability in comparison to cardiac MRI in post-STEMI patients.
Methods: 49 patients were prospectively enrolled in our 18-month’s study. Dobutamin stress echocardiography was performed 4 days post-infarction accompanied with automated functional imaging (Speckle tracking) analysis of left ventricle during rest and then during low dose stress. All patients underwent a follow up stress echocardiography at 6 weeks with speckle tracking analysis. Cardiac MRI took place concomitantly at 4 days post-infarction and 6 weeks. We carried out an assessment of re-admission with acute coronary syndrome (ACS) after one year of enrollment.
Results: Investigating strain rate obtained with stress speckle tracking after revascularization predicted the extent of myocardial scar, determined by contrast-enhanced magnetic resonance imaging. A good correlation was found between the global strain and total infarct size (R 0.75, p< 0.001). Furthermore, a clear inverse relationship was found between the segmental strain and the transmural extent of infarction in each segment. (R -0.69, p<0.01). Meanwhile it provided 81.82% sensitivity and 82.6% specificity to detect transmural from non-transmural infarction at a cut-off value of -10.15. Global stress strain rate showed 80% sensitivity and 77.5% specificity at a cut-off value of -9.1 to predict hospital re-admission with ACS. A cut-off value of -8.4 had shown a 69.23% sensitivity and 73.5% specificity to predict the re-admission related to other cardiac symptoms.
Conclusion: Strain rate obtained from speckle tracking during stress is a novel method of detecting myocardial viability after STEMI .Moreover it carries a promising role in post-myocardial infarction risk stratification with a reasonable prediction of reversible cardiac-related hospital re-admission.
Adipocytes are specialized cells found in vertebrates to ensure survival in terms of adaption to food deficit and abundance. However, their dysfunction accounts for the pathophysiology of metabolic diseases such as T2DM. Preliminary data generated by Mona Löffler suggested that PKD1 is involved in adipocyte function. Here, I show that PKD1 expression and activity is linked to lipid metabolism of murine adipocytes. PKD1 gene expression and activity was reduced in murine white adipose tissue upon fasting, a physiological condition which induces lipolysis. Isoproterenol-stimulated lipolysis in adipose tissue and 3T3-L1 adipocytes reduced PKD1 gene expression. Silencing ATGL in adipocytes inhibited isoproterenol-stimulated lipolysis, however, the β-adrenergic
stimulation of ATGL-silenced adipocytes lowered PKD1 expression levels as well. Adipose tissue of obese mice exhibited high PKD1 RNA levels but paradoxically lower protein levels of phosphorylated PKD1-Ser916. However, HFD generated a second
PKD1 protein product of low molecular weight in mouse adipose tissue. Furthermore, constitutively active PKD1 predominantly displayed nuclear localization in 3T3-L1 adipocytes containing many fat vacuoles. However, adipocytes
overexpressing non-functional PKD1 contained fewer lipid droplets and PKD1-KD was distributed in cytoplasm. Most importantly, deficiency of PKD1 in mouse adipose tissue caused expression of genes involved in adaptive thermogenesis such as UCP-1 and thus generated brown-like phenotype adipocytes. Thus, PKD1 is implicated in adipose tissue function and presents an interesting target for therapeutic approaches in the prevention of obesity and associated diseases.
Einleitung:
Multiple-Choice-Klausuren spielen immer noch eine herausragende Rolle für fakultätsinterne medizinische Prüfungen. Neben inhaltlichen Arbeiten stellt sich die Frage, wie die technische Abwicklung optimiert werden kann. Für Dozenten in der Medizin gibt es zunehmend drei Optionen zur Durchführung von MC-Klausuren: Papierklausuren mit oder ohne Computerunterstützung oder vollständig elektronische Klausuren. Kritische Faktoren sind der Aufwand für die Formatierung der Klausur, der logistische Aufwand bei der Klausurdurchführung, die Qualität, Schnelligkeit und der Aufwand der Klausurkorrektur, die Bereitstellung der Dokumente für die Einsichtnahme, und die statistische Analyse der Klausurergebnisse.
Methoden:
An der Universität Würzburg wird seit drei Semestern ein Computerprogramm zur Eingabe und Formatierung der MC-Fragen in medizinischen und anderen Papierklausuren verwendet und optimiert, mit dem im Wintersemester (WS) 2009/2010 elf, im Sommersemester (SS) 2010 zwölf und im WS 2010/11 dreizehn medizinische Klausuren erstellt und anschließend die eingescannten Antwortblätter automatisch ausgewertet wurden. In den letzten beiden Semestern wurden die Aufwände protokolliert.

Der Aufwand der Formatierung und der Auswertung einschl. nachträglicher Anpassung der Auswertung einer Durchschnittsklausur mit ca. 140 Teilnehmern und ca. 35 Fragen ist von 5-7 Stunden für Klausuren ohne Komplikation im WS 2009/2010 über ca. 2 Stunden im SS 2010 auf ca. 1,5 Stunden im WS 2010/11 gefallen. Einschließlich der Klausuren mit Komplikationen bei der Auswertung betrug die durchschnittliche Zeit im SS 2010 ca. 3 Stunden und im WS 10/11 ca. 2,67 Stunden pro Klausur.
Diskussion:
Für konventionelle Multiple-Choice-Klausuren bietet die computergestützte Formatierung und Auswertung von Papierklausuren einen beträchtlichen Zeitvorteil für die Dozenten im Vergleich zur manuellen Korrektur von Papierklausuren und benötigt im Vergleich zu rein elektronischen Klausuren eine deutlich einfachere technische Infrastruktur und weniger Personal bei der Klausurdurchführung.
In der vorliegenden Studie wurden Häufigkeit, Charakteristika und Einflussfaktoren von Überleben, funktioneller Kapazität, Atempumpenstörung und pulmonaler Hypertonie bei Sarkoidose analysiert. Insgesamt wurden die Daten von 182 mitteleuropäischen Patienten mit gesicherter Sarkoidose retrospektiv erfasst und der Überlebensstatus erhoben. Von 127 Patienten lagen die kompletten Ergebnisse der Diagnostik in der Missionsärztlichen Klinik Würzburg vor. Mit den Daten aller dieser Patienten wurden Überlebensanalysen, Prädiktionsanalysen und Korrelationsanalysen durchgeführt. Damit war die Studie eine der umfangreichsten, die sich mit mitteleuropäischen Sarkoidosepatienten und ihrem Krankheitsverlauf retrospektiv auseinandersetzte.
Von allen 179 im Follow-up erfassten Patienten sind 19 (11 männlich) im mittleren Alter von 68 Jahren verstorben (10%). Eine pulmonale Hypertonie wurde in 6 Fällen mittels Rechtsherzkatheter wenige Monate bis 3 Jahre vor dem Tod diagnostiziert und 7 verstorbene Patienten erfüllten die echokardiographischen Kriterien einer pulmonalen Hypertonie. Die Prävalenz der SAPH war insgesamt 10% (12 Patienten mit mPAP 39±10 mmHg), der echokardiographische Verdacht auf pulmonale Hypertonie bestand bei 22 (PASP > 36 mmHg) bzw. 15 (TRVmax > 2,8 m/s) Patienten. Funktionell relevant waren Rechtsherzfunktion (RA, TAPSE, NT-proBNP), Hämodynamik (PASP, TRVmax, mPAP), Ventilation (FEV1, VC) und Transferfaktor (r bzw. ß > 0,4 bzw. < -0,4, p < 0.05). Mortalitätsprädiktoren waren u.a. eine Gehstrecke < 338 m und eine V’O2peak ≤ 63,5 %/Soll (AUC > 0,8, p < 0.05). Bei 13 von 17 mit Mundverschlussdruckmessung untersuchten Patienten wurde eine Atempumpenstörung diagnostiziert. 7 der 13 Patienten hatten eine invasiv-diagnostizierte SAPH. Die 13 Patienten zeigten eine erhöhte Atempumpenlast (P0.1 0,56[0,38-0,66] kPa) und 12 der 13 Patienten auch eine erhöhte respiratorische Kapazität (P0.1/PImax 12,0[7,5-19,5]%) und Atemfrequenz (BF 23[19-24]/min). Die Atemmuskelkraft war nur bei einer Patientin pathologisch gemäß den Grenzwerten von Koch et al. Die Atempumpenlast bzw. die respiratorische Kapazität hing stark vom pulmonalarteriellen Druck ab (mPAP, PVR, PASP, TRVmax korrelierten sehr gut mit P0.1 bzw. P0.1/PImax). SAPH-Patienten und Patienten ohne pulmonale Hypertonie zeigten signifikante Unterschiede in Bezug auf Atemmuskelkraft und Atempumpenlast. Die echokardiographischen Lungenhochdruck-Parameter hatten starke prädiktive Eigenschaften (100% Sensitivität, Spezifität) bezüglich der Entwicklung einer Atempumpenstörung (PASP ≥ 40 mmHg, TRVmax ≥ 2,9 m/s). Innerhalb des Lungenhochdruck-Kollektivs war der Zusammenhang zwischen Atempumpenlast und Parametern der pulmonalen Hypertonie und der Einfluss auf funktionelle Kapazität und Ventilation zudem wesentlich stärker (r > 0,8 bzw. < -0,8, p < 0.05).
Die Prävalenz der SAPH ist hoch, das Auftreten mit eingeschränktem Überleben und funktioneller sowie Atempumpeneinschränkung assoziiert. Der Verlauf der Sarkoidose wird von dem Auftreten einer pulmonale Hypertonie, einer Atemlasterhöhung und einer Einschränkung der funktionellen Kapazität beeinflusst. Die bisher gefundenen Daten sollten an größeren Populationen bestätigt oder korrigiert werden. Sarkoidose-Mortalitäts- sowie SAPH-Prädiktoren könnten im klinischen Alltag in Form von Therapiekontroll- und Screeningparametern von Nutzen sein und das Management der Sarkoidosepatienten erleichtern.
Einleitung: Medizinische Trainingsfälle sind in der studentischen Ausbildung inzwischen weit verbreitet. In den meisten Publikationen wird über die Entwicklung und die Erfahrungen in einem Kurs mit Trainingsfällen berichtet. In diesem Beitrag vergleichen wir die Akzeptanz von verschiedenen Trainingsfallkursen, die als Ergänzung zu zahlreichen Vorlesungen der Medizinischen Fakultät der Universität Würzburg mit sehr unterschiedlichen Nutzungsraten eingesetzt wurden, über einen Zeitraum von drei Semestern.
Methoden: Die Trainingsfälle wurden mit dem Autoren- und Ablaufsystem CaseTrain erstellt und über die Moodle-basierte Würzburger Lernplattform WueCampus den Studierenden verfügbar gemacht. Dabei wurden umfangreiche Daten über die Nutzung und Akzeptanz erhoben.
Ergebnisse: Im Zeitraum vom WS 08/09 bis zum WS 09/10 waren 19 Kurse mit insgesamt ca. 200 Fällen für die Studierenden verfügbar, die pro Semester von ca. 550 verschiedenen Medizinstudenten der Universität Würzburg und weiteren 50 Studierenden anderer bayerischer Universitäten genutzt wurden. Insgesamt wurden pro Semester ca. 12000 Mal Trainingsfälle vollständig durchgespielt zu denen ca. 2000 Evaluationen von den Studierenden ausgefüllt wurden. In den verschiedenen Kursen variiert die Nutzung zwischen unter 50 Bearbeitungen in wenig frequentierten Fallsammlungen und über 5000 Bearbeitungen in stark frequentierten Fallsammlungen.
Diskussion: Auch wenn Studierende wünschen, dass zu allen Vorlesungen Trainingsfälle angeboten werden, zeigen die Daten, dass der Umfang der Nutzung nicht primär von der Qualität der verfügbaren Trainingsfälle abhängt. Dagegen werden die Trainingsfälle in fast allen Fallsammlungen kurz vor den Klausuren extrem häufig bearbeitet. Dies zeigt, dass die Nutzung von Trainingsfällen im Wesentlichen von der wahrgenommenen Klausurrelevanz der Fälle abhängt.
Kürzlich wurden bei immunvermittelten Neuropathien Autoantikörper gegen Proteine
des paranodalen axoglialen Komplexes beschrieben. Deren Charakteristika,
Prävalenzen, pathophysiologische Relevanz sowie Bedeutung für Diagnostik
und Therapie sind jedoch noch nicht abschließend erforscht.
In dieser Studie wurden daher Seren und Plasmapheresematerial (PE-Material)
von 150 Patienten mit inflammatorischen Neuropathien, nämlich 105 mit chronisch
inflammatorischer demyelinisierender Polyneuropathie (CIDP), 21 mit Guillain-
Barré-Syndrom (GBS) und 24 mit multifokaler motorischer Neuropathie
(MMN), welche etablierte diagnostische Kriterien der jeweiligen Krankheit erfüllen,
sowie 74 Kontrollen mittels immunhistochemischen Färbungen an murinen
Zupfnervenpräparaten und/oder ELISA (Enzyme-linked Immunosorbent Assay)
auf Autoantikörper gegen die paranodalen Proteine Caspr, Contactin-1 und Neurofascin-
155 untersucht. Bei positivem Ergebnis wurde deren Spezifität mittels
immunhistochemischen Färbungen an transfizierten HEK (Human embryonic kidney)-
293-Zellen und Präinkubationsversuchen bestätigt. Es wurden die IgG-Subklassen
und die Antikörpertiter bestimmt und das Komplementbindungsverhalten
unter Zugabe von intravenösen Immunglobulinen (IVIG) mit zellbasierten und
ELISA-basierten Methoden analysiert. Klinische Merkmale und das Therapieansprechen
Antikörper-positiver Patienten wurden ermittelt und mit den experimentellen
Ergebnissen in Zusammenhang gesetzt.
IgG-Autoantikörper gegen Contactin-1 konnten bei vier Patienten mit CIDP nachgewiesen
werden, IgG-Autoantikörper gegen Caspr bei einem Patienten mit
CIDP und einer Patientin mit GBS. Es konnten keine weiteren Autoantikörper bei
CIDP-Patienten, GBS-Patienten, MMN-Patienten oder bei den Kontrollen detektiert
werden. Die Prävalenz von Autoantikörpern gegen axogliale paranodale Proteine
liegt somit in dieser Studie bei jeweils 4,76% bei CIDP und GBS und 0%
bei MMN. Die Antikörper gehörten bei Patienten in der akuten Erkrankungsphase
(zwei der CIDP-Patienten mit Anti-Contactin-1-Autoantikörpern und eine GBS-Patientin mit Anti-Caspr-Autoantikörpern) hauptsächlich den Subklassen IgG1
und IgG3 an, bei Patienten in der chronischen Phase (zwei der CIDP-Patienten
mit Anti-Contactin-1-Autoantikörpern, ein CIDP-Patient mit Anti-Caspr-Autoantikörpern)
überwog die Subklasse IgG4. Experimentell kam es zur Komplementbindung
und -aktivierung abhängig vom Gehalt der Subklassen IgG1-3, nicht
aber IgG4; diese konnte durch die Zugabe von IVIG dosisabhängig gemindert
werden. Alle Autoantikörper-positiven CIDP-Patienten zeigten einen GBS-artigen
Beginn mit einer schweren motorischen Beteiligung. Anti-Contactin-1-positive
Patienten kennzeichnete klinisch zusätzlich das Vorkommen einer Ataxie und eines
Tremors, Anti-Caspr-positive Patienten das Vorkommen starker neuropathischer
Schmerzen. Elektrophysiologisch standen neben Hinweisen auf eine Leitungsstörung
Zeichen einer axonalen Schädigung im Vordergrund. Als histopathologisches
Korrelat lagen eine nodale Architekturstörung und ein Axonverlust
vor. Die Patienten zeigten nur in der Anfangsphase der Erkrankung ein Ansprechen
auf IVIG. Bei drei CIDP-Patienten mit IgG4-Autoantikörpern (zwei Patienten
mit Anti-Contactin-1-Antikörpern und ein Patient mit Anti-Caspr-Antikörpern)
wurde eine Therapie mit Rituximab durchgeführt. Diese führte zu einer Titerreduktion
und zur zeitgleichen klinischen und elektrophysiologischen Befundbesserung
bei zwei Patienten.
Die in dieser Arbeit angewandten Screeningmethoden führten zum erfolgreichen
Nachweis von Autoantikörpern gegen paranodale axogliale Proteine. Die Patienten
mit positivem Autoantikörpernachweis definieren eine kleine Untergruppe mit
ähnlichen klinischen Merkmalen im Kollektiv der Patienten mit inflammatorischen
Polyneuropathien. Histopathologische Merkmale sowie das Therapieansprechen
auf antikörperdepletierende Therapie sprechen in Kombination mit den Ergebnissen
weiterer Studien zu paranodalen Autoantikörpern für eine pathogenetische
Relevanz der Autoantikörper. Mit einem charakteristischen, am Schnürring ansetzenden
Pathomechanismus könnten Neuropathien mit Nachweis von paranodalen
Autoantikörpern der kürzlich eingeführten Entität der Nodo-Paranodopathien
angehören. Die Komplementaktivierung und das Therapieansprechen der Patienten auf IVIG stehen möglicherweise in Zusammenhang mit der prädominanten
IgG-Subklasse. Diese könnte auch in Bezug auf die Chronifizierung eine
Rolle spielen. Der Nachweis von Autoantikörpern gegen paranodale Proteine hat
wohlmöglich in Zukunft direkte Konsequenzen auf das diagnostische und therapeutische
Prozedere bei Patienten mit CIDP und GBS; weitere klinische und experimentelle
Daten aus größeren, prospektiven Studien sind jedoch zum weiteren
Verständnis und zur Charakterisierung dieser Entität notwendig.
Bacterial functional membrane microdomains (FMMs) are membrane platforms that resemble lipid rafts of eukaryotic cells in certain functional and structural aspects. Lipid rafts are nanometer-sized, dynamic clusters of proteins and lipids in eukaryotic cell membranes that serve as signaling hubs and assembling platforms. Yet, studying these structures can often be hampered by the complexity of a eukaryotic cell. Thus, the analogous structures of prokaryotes are an attractive model to study molecular traits of this type of membrane organization.
Similar to eukaryotic lipid rafts, the bacterial FMMs are comprised of polyisoprenoid lipids, scaffold proteins and a distinct set of membrane proteins, involved in signaling or secretion. Investigating bacterial FMMs not only contributes to the understanding of the physiological importance of FMMs in bacteria, but also helps to elucidate general principles of rafts beyond prokaryotes.
In this work, a bacterial model organism was used to investigate effects of synthetic overproduction of the raft scaffolding proteins on bacterial physiology. This overexpression causes an unusual stabilization of the FMM-harbored protease FtsH and therefore the proteolytic targets of FtsH are not correctly regulated. Developmental defects and aberrances in shape are the consequence, which in turn negatively affects cell physiology. These findings may be adapted to better understand lipid raft processes in humans, where flotillin upregulation is detected along with development of neurological diseases.
Moreover, it was aimed at understanding the FMM-proteome of the human pathogen Staphylococcus aureus. An in-depth quantitative mass-spectrometry analysis reveals adaption of the protein cargo during different conditions, while maintaining a distinct set of core FMM proteins. As a case study, the assembly of the type VII secretion system was shown to be dependent on FMM integrity and more specifically on the activity of the FMM-scaffold flotillin. This secretion system is important for the virulence of this pathogen and its secretion efficiency can be targeted by small molecules that inhibit flotillin activity. This opens new venues for non-conventional antimicrobial compounds to treat staphylococcal infections.
Eine ausgeprägte Mundtrockenheit, Xerostomie, entsteht häufig durch eine irreversible Funktionseinschränkung der Speicheldrüsen. Diese ist unter anderem durch die Einnahme bestimmter Medikamente, Autoimmunerkrankungen, fortgeschrittenes Alter oder die Bestrahlungstherapie von Tumoren der Kopf-Hals-Region bedingt, wobei letztere eine der häufigsten Ursachen darstellt. Konsequenzen der eingeschränkten Drüsenfunktion sind herabgesetzte Speichelflussraten, eine Reduktion des Mund-pH-Werts, eine veränderte Elektrolyt- und Immunglobulin-Zusammensetzung des Speichels und somit eine Verringerung des Infektionsschutzes. Die resultierenden Komplikationen erstrecken sich von Karies und rezidivierenden Infektionen bis hin zu Pilzbesiedelungen der Mundschleimhaut. Diese schränken die Lebensqualität der Patienten stark ein und führen häufig zu Therapieunterbrechungen. Fast die Hälfte der Patienten leidet unter Depressionen oder psychischen Belastungszuständen. Es gibt wenige Therapieansätze zur Behandlung der postradiogenen Xerostomie: Pilocarpin erhöht zwar die Speichelflussraten, hat jedoch keinen signifikanten Effekt auf die Lebensqualität. Die operative Translokation der Glandula submandibularis hat den Weg in die klinische Routine noch nicht gefunden, während die intensitätsmodulierte Bestrahlung (IMRT) nicht für jeden Patienten geeignet ist; beide zeigen jedoch einen positiven Effekt auf die Lebensqualität. Gentechnische und stammzellbasierte Ansätze zur Regeneration des Drüsengewebes befinden sich im Experimentalstadium. Somit ergibt sich ein dringender Bedarf an innovativen Optionen zur Behandlung der postradiogenen Xerostomie. Das Tissue Engineering, die Erstellung einer künstlichen Speicheldrüse aus körpereigenen Zellen, böte hier ein potentielles Behandlungskonzept.
Diese Studie soll deshalb untersuchen, ob humane Speicheldrüsenepithelzellen (hSEZ) auf einer Matrix aus dezellularisiertem, porzinem Jejunum, der sogenannten Small intestinal submucosa + mucosa (SIS-muc), kultiviert werden können. Können die Zellen innerhalb der Wachstumsperiode wichtige physiologische Differenzierungsmarker beibehalten? Kann die Produktion von α-Amylase, einem der wichtigsten Enzyme des menschlichen Speichels, erhalten werden? Welchen Einfluss hat die Kokultur mit mikrovaskulären Endothelzellen (mvEZ)? Und zuletzt: Ist dezellularisierter Schweinedarm eine potentiell geeignete Matrix für das Tissue Engineering der menschlichen Speicheldrüse?
Zunächst erfolgte die Entnahme von humanem Speicheldrüsengewebe, woraus hSEZ isoliert wurden. Diese wurden dann sowohl in Mono- als auch in Kokultur mit mvEZ auf die SIS-muc aufgebracht und auf dieser kultiviert. Die SIS-muc wurde aus kurzen Schweinedarm-Segmenten gewonnen, die in einem mehrstufigen Verfahren dezellularisiert wurden. Die besiedelte SIS-muc wurde mittels konventioneller sowie Immunfluoreszenzfärbungen, Raster- und Transmissionsektronenmikroskopie (REM/TEM) sowie quantitativer Polymerasekettenreaktion (qPCR) untersucht, darüber hinaus erfolgte die Messung der α-Amylase-Enzymaktivität.
Histologisch sowie in der REM zeigte sich sowohl in der Mono- als auch in der Kokultur eine konfluente Besiedelung der SIS-muc mit hSEZ. In der Kokultur formten mvEZ einen Monolayer auf der serosalen Matrixseite. Bei der Charakterisierung der hSEZ zeigte sich in den Immunfluoreszenzaufnahmen eine starke Ausprägung von Zytokeratin, α-Amylase und Aquaporin-5 und eine moderate Ausprägung von Claudin-1. Bei der Untersuchung der Funktion der α-Amylase konnte in der Kokultur von hSEZ mit mvEZ eine im Gegensatz zur Mono- und 2D-Kultur signifikant erhöhte Enzymaktivität der α-Amylase nachgewiesen werden. In der qPCR-Analyse der α-Amylase-Genexpression war die 3D-Kultur der 2D-Kultur überlegen.
Die vorliegende Arbeit zeigt, dass die Kultur von hSEZ auf der SIS-muc möglich ist. Es konnte nachgewiesen werden, dass die Zellen in 3D-Kultur spezifische Differenzierungsmerkmale beibehalten, die in der 2D-Kultur teils verloren gehen und dass hSEZ in Kokultur mit mvEZ eine gegenüber der Monokultur signifikant erhöhte Produktion von α-Amylase aufweisen. Diese Arbeit liefert die Datengrundlage für zukünftige Studien im dynamischen Bioreaktor-Modell (BioVaSc), die auf dem Weg zur klinischen Translation notwendig sind. Somit stellt sie einen wichtigen Schritt in Richtung einer auf Tissue Engineering basierten Therapie der belastenden Xerostomie dar.
The human-bacterial pathogen interaction is a complex process that results from
a prolonged evolutionary arms race in the struggle for survival. The pathogen employs
virulence strategies to achieve host colonization, and the latter counteracts using defense
programs. The encounter of both organisms results in drastic physiological changes
leading to stress, which is an ancient response accompanying infection. Recent evidence
suggests that the stress response in the host converges with the innate immune pathways
and influences the outcome of infection. However, the contribution of stress and the exact
mechanism(s) of its involvement in host defense remain to be elucidated. Using the model
bacterial pathogen Shigella flexneri, and comparing it with the closely related pathogen
Salmonella Typhimurium, this study investigated the role of host stress in the outcome of
infection.
Shigella infection is characterized by a pronounced pro-inflammatory response
that causes intense stress in host tissues, particularly the intestinal epithelium, which
constitutes the first barrier against Shigella colonization. In this study, inflammatory
stress was simulated in epithelial cells by inducing oxidative stress, hypoxia, and cytokine
stimulation. Shigella infection of epithelial cells exposed to such stresses was strongly
inhibited at the adhesion/binding stage. This resulted from the depletion of sphingolipidrafts
in the plasma membrane by the stress-activated sphingomyelinases. Interestingly,
Salmonella adhesion was not affected, by virtue of its flagellar motility, which allowed the
gathering of bacteria at remaining membrane rafts. Moreover, the intracellular replication
of Shigella lead to a similar sphingolipid-raft depletion in the membrane across adjacent
cells inhibiting extracellular bacterial invasion.
Additionally, this study shows that Shigella infection interferes with the host stress
granule-formation in response to stress. Interestingly, infected cells exhibited a nuclear
depletion of the global RNA-binding stress-granule associated proteins TIAR and TIA-1
and their accumulation in the cytoplasm.
Overall, this work investigated different aspects of the host stress-response in the
defense against bacterial infection. The findings shed light on the importance of the host
stress-pathways during infection, and improve the understanding of different strategies
in host-pathogen interaction.
Humans are continuously exposed to airborne spores of the saprophytic fungus Aspergillus fumigatus. In healthy individuals, local pulmonary host defence mechanisms can efficiently eliminate the fungus without any overt symptoms. In contrast, A. fumigatus causes devastating infections in immunocompromised patients. However, local host immune responses against A. fumigatus lung infections in immunocompromised conditions have remained largely elusive.
Given the dynamic changes in immune cell subsets within tissues upon immunosuppressive therapy, we dissected the spatiotemporal pulmonary immune response after A. fumigatus infection to reveal basic immunological events that fail to effectively control the invasive fungal disease. In different immunocompromised murine models, myeloid but not lymphoid cells were strongly recruited upon infection. Notably, neutrophils and macrophages were recruited to infected lungs in different immunosuppressed regimens. Other myeloid cells, particularly dendritic cells and monocytes were only recruited in the corticosteroid model after infection. Lymphoid cells, particularly CD4+ or CD8+ T-cells and NK cells were highly reduced upon immunosuppression and were not recruited after A. fumigatus infection. Importantly, adoptive CD11b+ myeloid cell transfer rescued immunosuppressed mice from lethal A. fumigatus infection. These findings illustrate that CD11b+ myeloid cells are critical for anti-A. fumigatus defence under immunocompromised conditions.
Despite improved antifungal agents, invasive A. fumigatus lung infections cause a high rate morbidity and mortality in neutropenic patients. Granulocyte transfusions have been tested as an alternative therapy for the management of high-risk neutropenic patients with invasive A. fumigatus infections. To increase the granulocyte yield for transfusion, donors are treated with corticosteroids. Yet, the efficacy of granulocyte transfusion and the functional defence mechanisms of granulocytes collected from corticosteroid treated donors remain largely elusive.
We aimed to assess the efficacy of granulocyte transfusion and functional defence mechanisms of corticosteroid treated granulocytes using mouse models.
In this thesis, we show that transfusion of granulocytes from corticosteroid treated mice did not protect cyclophosphamide immunosuppressed mice against lethal A. fumigatus infection in contrast to granulocytes from untreated mice. Upon infection, increased levels of inflammatory cytokines helped to recruit granulocytes to the lungs without any recruitment defects in corticosteroid treated and infected mice or in cyclophosphamide immunosuppressed and infected mice that have received the granulocytes from corticosteroid treated mice. However, corticosteroid treated human or mouse neutrophils failed to form neutrophil extracellular traps (NETs) in in vitro and in vivo conditions. Further, corticosteroid treated granulocytes exhibited impaired ROS production against A. fumigatus. Notably, corticosteroids impaired the β-glucan receptor Dectin-1 (CLEC7A) on mouse and human granulocytes to efficiently recognize and phagocytize A. fumigatus, which markedly impaired fungal killing. We conclude that corticosteroid treatment of granulocyte donors for increasing neutrophil yields or patients with ongoing corticosteroid treatment could result in deleterious effects on granulocyte antifungal functions, thereby limiting the benefit of granulocyte transfusion therapies against invasive fungal infections.
Obese children and adolescents are at high risk of developing cardiovascular diseases later in life. We hypothesized that cardiovascular prophylaxis with omega-3 fatty acids could benefit them. In our study, 20 children and adolescents (mean body mass index percentile: 99.1; mean age: 11.0 years) underwent two ambulatory 24 h Holter electrocardiography (ECG) recordings (before and after at least 3 months of omega-3 fatty acid supplementation). Time domain heart rate variability (HRV) and heart rate (HR) were examined for these patients. As a control, we used 24 h Holter ECG recordings of 94 nonobese children and adolescents. Time domain HRV parameters, which are indicators of vagal stimulation, were significantly lower in obese patients than in healthy controls, but HR was higher (standard deviation of the normal-to-normal [SDNN] interbeat intervals: −34.02%; root mean square of successive differences [RMSSD] between normal heartbeats: −40.66%; percentage of consecutive RR intervals [pNN50]: −60.24%; HR: +13.37%). After omega-3 fatty acid supplementation, time domain HRV parameters and HR of obese patients were similar to the values of healthy controls (SDNN interbeat intervals: −21.73%; RMSSD: −19.56%; pNN50: −25.59%; HR: +3.94%). Therefore, omega-3 fatty acid supplementation may be used for cardiovascular prophylaxis in obese children and adolescents.
Background:
Sleep-related eating may occur in the context of mental illness, sleep disorders, or psychopharmacological treatment. Frequently, sleep-related eating leads to severe weight gain and, so far, there are no treatment options for the condition.
Case presentation:
We report the case of a 54-year-old white woman with depression, panic disorder, and sleep apnea under treatment with various antidepressants who developed severe sleep-related eating. Her sleep-related eating completely vanished after addition of agomelatine, it reoccurred after cessation of agomelatine, and vanished again after her re-exposure to another melatonergic drug, extended melatonin.
Conclusions:
This case suggests that melatonergic drugs lead to relief from sleep-related eating, even when the condition occurs in the context of physical and mental disorders as well as psychopharmacological treatment.
Die vorliegende Arbeit überprüft die Entlassungsbriefe des ersten Halbjahres 2011 der Medizinischen Klinik II des Klinikums Aschaffenburg – Alzenau. Ziel der Arbeit ist es zu erfassen, wie viele Patienten mit einer PPI-Medikation aus der stationären Behandlung entlassen wurden und in wie vielen Fällen diese Behandlung indikations-, zeit- und dosisgerecht war. Es sollte damit auch überprüft werden, ob in Entlassungsbriefen einer gastroenterologischen Fachabteilung ein höheres Bewusstsein für eine leitlinienkonforme PPI-Empfehlung vorliegt.
Die Haupt- Fragestellung der Arbeit war: Welchen Einfluss hat Kinesiotape in der ersten postoperativen Phase?
Dazu wurde versucht eine Vergleichbarkeit der Gruppen herzustellen, indem Einflussfaktoren wie Alter, körperliche Voraussetzungen, Geschlecht, PT Behandlung und Schmerzmittelbedarf verglichen wurden.
Durch die Unterscheidung der Gruppen anhand von: Beweglichkeit des Kniegelenkes, Änderung d. Beweglichkeit, Umfangmessung des Kniegelenks, sowie des Oberschenkels und Schmerzentwicklung sollten Rückschlüsse auf das Kinesiotape gezogen werden

Background
Human African Trypanosomiasis (HAT) also called sleeping sickness is an infectious disease in humans caused by an extracellular protozoan parasite. The disease, if left untreated, results in 100% mortality. Currently available drugs are full of severe drawbacks and fail to escape the fast development of trypanosoma resistance. Due to similarities in cell metabolism between cancerous tumors and trypanosoma cells, some of the current registered drugs against HAT have also been tested in cancer chemotherapy. Here we demonstrate for the first time that the simple ester, ethyl pyruvate, comprises such properties.
Results
The current study covers the efficacy and corresponding target evaluation of ethyl pyruvate on T. brucei cell lines using a combination of biochemical techniques including cell proliferation assays, enzyme kinetics, phasecontrast microscopic video imaging and ex vivo toxicity tests. We have shown that ethyl pyruvate effectively kills trypanosomes most probably by net ATP depletion through inhibition of pyruvate kinase (Ki = 3.0\(\pm\)0.29 mM). The potential of ethyl pyruvate as a trypanocidal compound is also strengthened by its fast acting property, killing cells within three hours post exposure. This has been demonstrated using video imaging of live cells as well as concentration and time dependency experiments. Most importantly, ethyl pyruvate produces minimal side effects in human red cells and is known to easily cross the blood-brain-barrier. This makes it a promising candidate for effective treatment of the two clinical stages of sleeping sickness. Trypanosome drug-resistance tests indicate irreversible cell death and a low incidence of resistance development under experimental conditions.
Conclusion
Our results present ethyl pyruvate as a safe and fast acting trypanocidal compound and show that it inhibits the enzyme pyruvate kinase. Competitive inhibition of this enzyme was found to cause ATP depletion and cell death. Due to its ability to easily cross the blood-brain-barrier, ethyl pyruvate could be considered as new candidate agent to treat the hemo-lymphatic as well as neurological stages of sleeping sickness.
Biological systems such as cells or whole organisms are governed by complex regulatory networks of transcription factors, hormones and other regulators which determine the behavior of the system depending on internal and external stimuli. In mathematical models of these networks, genes are represented by interacting “nodes” whose “value” represents the activity of the gene.
Control processes in these regulatory networks are challenging to elucidate and quantify. Previous control centrality metrics, which aim to mathematically capture the ability of individual nodes to control biological systems, have been found to suffer from problems regarding biological plausibility.
This thesis presents a new approach to control centrality in biological networks. Three types of network control are distinguished: Total control centrality quantifies the impact of gene mutations and identifies potential pharmacological targets such as genes involved in oncogenesis (e.g. zinc finger protein GLI2 or bone morphogenetic proteins in chondrocytes). Dynamic control centrality describes relaying functions as observed in signaling cascades (e.g control in mouse colon stem cells). Value control centrality measures the direct influence of the value of the node on the network (e.g. Indian hedgehog as an essential regulator of proliferation in chondrocytes). Well-defined network manipulations define all three centralities not only for nodes, but also for the interactions between them, enabling detailed insights into network pathways.
The calculation of the new metrics is made possible by substantial computational improvements in the simulation algorithms for several widely used mathematical modeling paradigms for genetic regulatory networks, which are implemented in the regulatory network simulation framework Jimena created for this thesis.
Applying the new metrics to biological networks and artificial random networks shows how these mathematical concepts correspond to experimentally verified gene functions and signaling pathways in immunity and cell differentiation. In contrast to controversial previous results even from the Barabási group, all results indicate that the ability to control biological networks resides in only few driver nodes characterized by a high number of connections to the rest of the network. Autoregulatory loops strongly increase the controllability of the network, i.e. its ability to control itself, and biological networks are characterized by high controllability in conjunction with high robustness against mutations, a combination that can be achieved best in sparsely connected networks with densities (i.e. connections to nodes ratios) around 2.0 - 3.0.
The new concepts are thus considerably narrowing the gap between network science and biology and can be used in various areas such as system modeling, plausibility trials and system analyses.
Medical applications discussed in this thesis include the search for oncogenes and pharmacological targets, as well their functional characterization.
Objective: To examine the effects of two different treatment approaches on the course of anorexia nervosa (AN) over time.
Methods: The subjects were 27 hospitalized AN patients (mean age: 14.91 years; mean BMI: 14.58; mean height: 163.56) . In our retrospective analysis we compared weight gain in two groups. While one group was treated with a standard oral refeeding protocol (historical control) through January 2013 (N=16), the second group (highly standardized refeeding protocol) received a high energy liquid nutrition and nutritional supplements including omega-3 fatty acids (N=11).
Results: On admission, the two groups were comparable in terms of height, weight, age and heart rate. At the end of our monitoring time frame of 25 days, weight gain was 121.4% higher in the highly standardized refeeding protocol group than in the historical control group (66.5 ±52.4 vs 147.3 ±55.7 grams/day; t-Test p=0.004; CI95%: 29.3-132.2). About 45% of our patients stated they were vegetarians at admission. However, we could not identify a vegetarian diet as a statistically significant negative prognostic factor for weight gain.
Discussion: The highly standardized refeeding protocol seems to be helpful in malnourished AN patients to improve weight gain without enhancing the risk of a refeeding syndrome. Because of an increasing energy turnover, caloric intake should be adjusted during refeeding.
Background: Perfusion-cardiovascular magnetic resonance (CMR) is generally accepted as an alternative to SPECT to assess myocardial ischemia non-invasively. However its performance vs gated-SPECT and in sub-populations is not fully established. The goal was to compare in a multicenter setting the diagnostic performance of perfusion-CMR and gated-SPECT for the detection of CAD in various populations using conventional x-ray coronary angiography (CXA) as the standard of reference.
Methods: In 33 centers (in US and Europe) 533 patients, eligible for CXA or SPECT, were enrolled in this multivendor trial. SPECT and CXA were performed within 4 weeks before or after CMR in all patients. Prevalence of CAD in the sample was 49% and 515 patients received MR contrast medium. Drop-out rates for CMR and SPECT were 5.6% and 3.7%, respectively (ns). The study was powered for the primary endpoint of non-inferiority of CMR vs SPECT for both, sensitivity and specificity for the detection of CAD (using a single-threshold reading), the results for the primary endpoint were reported elsewhere. In this article secondary endpoints are presented, i.e. the diagnostic performance of CMR versus SPECT in subpopulations such as multi-vessel disease (MVD), in men, in women, and in patients without prior myocardial infarction (MI). For diagnostic performance assessment the area under the receiver-operator-characteristics-curve (AUC) was calculated. Readers were blinded versus clinical data, CXA, and imaging results.
Results: The diagnostic performance (= area under ROC = AUC) of CMR was superior to SPECT (p = 0.0004, n = 425) and to gated-SPECT (p = 0.018, n = 253). CMR performed better than SPECT in MVD (p = 0.003 vs all SPECT, p = 0.04 vs gated-SPECT), in men (p = 0.004, n = 313) and in women (p = 0.03, n = 112) as well as in the non-infarct patients (p = 0.005, n = 186 in 1-3 vessel disease and p = 0.015, n = 140 in MVD).
Conclusion: In this large multicenter, multivendor study the diagnostic performance of perfusion-CMR to detect CAD was superior to perfusion SPECT in the entire population and in sub-groups. Perfusion-CMR can be recommended as an alternative for SPECT imaging.
Background: Sub-Saharan Africa has a high prevalence of hepatitis B virus (HBV) infections. Health care workers (HCWs) are at high risk of contracting HBV infection through their occupation. Vaccination of HCWs against HBV is standard practice in many countries, but is often not implemented in resource-poor settings. We aimed with this cross-sectional study to determine HBV prevalence, HCW vaccination status, and the risk factors for HCWs contracting HBV infection in Tanzania.
Methods: We enrolled 600 HCWs from a tertiary Tanzanian hospital. Their demographics, medical histories, HBV vaccination details and risk factors for contracting blood-borne infections were collected using a standardized questionnaire. Serum samples were tested for HBV and hepatitis C virus (HCV) markers by ELISA techniques, PCR and an anti-HBs rapid test. HCWs were divided in two subgroups: those at risk of contracting HBV (rHCW 79.2 %) via exposure to potentially infectious materials, and those considered not at risk of contracting HBV (nrHCW, 20.8 %). Results: The overall prevalence of chronic HBV infection (HBsAg+, anti-HBc+, anti-HBs-) was 7.0 % (42/598). Chronic HBV infection was found in 7.4 % of rHCW versus 5.6 % of nrHCW(p-value = 0.484). HCWs susceptible to HBV (HBsAg-, anti-HBc-, anti-HBs-) comprised 31.3 %. HBV immunity achieved either by healed HBV infection (HBsAg-, anti-HBc+, anti-HBs+) or by vaccination (HBsAg-, anti-HBc-, anti-HBs+) comprised 36.5 % and 20.2 %, respectively. 4.8 % of participants had indeterminate results (HBsAg-, anti-HBc+, anti-HBc-IgM-, anti-HBs-). Only 77.1 % of HCWs who received a full vaccination course had an anti-HBs titer > 10 ml/U. An anti-HBs point-of-care test was 80.7 % sensitive and 96.9 % specific. There was a significantly higher risk for contracting HBV (anti-HBc+) among those HCW at occupational risk (rHCW) of older age (odds ratios (OR) in rHCW 3.297, p < 0.0001 vs. nrHCW 1.385, p = 0.606) and among those HCW being employed more than 11 years (OR 2.51, p < 0.0001***). HCV prevalence was low (HCV antibodies 1.2 % and HCV-RNA 0.3 %).
Conclusions: Chronic HBV infection is common among Tanzanian HCWs. One third of HCWs were susceptible to HBV infection, highlighting the need for vaccination. Due to high prevalence of naturally acquired immunity against HBV pre-testing might be a useful tool to identify susceptible individuals.
Das Ziel der Arbeit war zu untersuchen, ob der Stoffwechsel kolorektaler Karzi-nomzellen geeignete Targetstrukturen für mögliche therapeutische Ansätze aufweist. In Krebszellen induziert sowohl der Warburg-Effekt bei Normoxie als auch die anaerobe Glykolyse bei Hypoxie eine massive Bildung von Laktat. Wird die Krebszelle dauerhaft daran gehindert, die für die Glykolyse notwendi-gen Reduktionsäquivalente NADH+H+ mit Hilfe der Laktatdehydrogenase zu reoxidieren und/oder Laktat über die Transporter MCT1 und MCT4 nach außen zu schleusen, dann löst diese Kombination aus Mangelsituation und intrazellulärer Ansäuerung den apoptotischen Zelltod aus. Für die Situation in vivo ist entscheidend, dass auch Zellen von Normalgeweben zwar Laktat in Hypoxie bilden, dies jedoch keine vorherrschende physiologische Situation darstellt.
Die Hemmstoffe Natriumoxamat (NaOx) für die Laktatdehydrogenase und α-Cyano-4-Hydroxycinnamat (αCHC) für MCT1 und MCT4 wurden an den sechs humanen kolorektalen Karzinomzelllinien Colo741, HCT116, HT29, LS174T, SW620 und WiDr untersucht. Zusätzlich wurde der Glukoseverbrauch und die Laktatbildung bestimmt und die Funktion der Atmungskette überprüft. Die IC50-Werte für 5-FU, NaOx und αCHC wurden bestimmt und danach NaOx in einer Konzentration von 40x10-3 mol/L, αCHC in einer Konzentration von 2x10-3 mol/L und 5-FU in einer Konzentration von 5x10-6 mol/L eingesetzt. Die Zellen wurden bei tumorphysiologischen Sauerstoffkonzentrationen von 5 % und 1 % Sauerstoff für bis zu 120 Stunden inkubiert.
Die Funktion der Atmungskette in den Mitochondrien der kolorektalen Karzi-nomzellen wurde u. a. durch Bestimmung wichtiger Kenngrößen wie dem P:O Quotienten und des respiratorischen Kontrollindex (RKI) nachgewiesen. Fünf der sechs Karzinomzelllinien wiesen im Vergleich zur Kontrollzelllinie J774 einen verringerten P:O-Quotienten und respiratorischen Kontrollindex (RKI) auf, was darauf hindeutet, dass die Funktion der Mitochondrien dieser Zellen im Vergleich zu Kontrollzellen zwar verringert war, aber nicht vollständig aufgehoben. Dieses Ergebnis stützt die allgemein akzeptierte Auffassung, dass die meisten Tumore über funktionelle Mitochondrien verfügen.
Durch die Analyse des Glukosestoffwechsels wurden die sechs kolorektalen Zelllinien, die einen unterschiedlich stark ausgeprägten glykolytischen Phänotyp aufwiesen, nach der Stärke der Laktatbildung bei 5 % Sauerstoff in drei Kategorien eingeordnet. Zudem wurde für jede der sechs Zelllinien die Expression von LDH-A, LDH-B sowie MCT-1 und MCT-4 auf Proteinebene nachgewiesen.
Wesentliches Ziel der Untersuchungen war die Überprüfung des antiprolife-rativen Potentials der beiden Inhibitoren NaOx und αCHC einzeln oder in Kombination mit 5-FU bei den tumorspezifischen Sauerstoffkonzentrationen von 5 % und 1 %. Die Kombination aus NaOx und αCHC induzierte bei 1 % Sauerstoff nach 9 Tagen in Kultur zytotoxische Effekte und war damit so wirksam wie 5x10-6 mol/L 5-FU. Die Zugabe von 5-FU zur Kombination aus NaOx und αCHC führte zu keiner Steigerung des zelltoxischen Effektes. Die beiden Inhibitoren NaOx und αCHC waren für SW620 Zellen weniger wirksam als für Zellen der anderen fünf Zelllinien. Das mehr „oxidative“ Profil von SW620 Zellen (bester P:O-Quotient, geringste Laktatbildung bei 5 % und 1 % Sauerstoff; zudem die höchsten IC50-Werte für NaOx und αCHC) könnte erklären, warum die beiden Stoffwechselinhibitoren, die einen glykolytischen Phänotyp (starke Bildung von Laktat) erfordern, für SW620 Zellen von geringerer Wirksamkeit waren.
Für die Hemmstoffe NaOx und αCHC wurden zytostatische bzw. zytotoxische Effekte in kolorektalen Karzinomzellen gezeigt. Dies deutet darauf hin, dass Krebszellen auf einen ungehinderten glykolytischen Stoffwechsel angewiesen sind. Für beide Hemmstoffe wurde ebenfalls gezeigt, dass sie auch bei tumorre-levanten Sauerstoffkonzentrationen von 5 % und 1 % wirksam sind.
In North Korea, the prevalence of hepatitis B is high due to natural factors, gaps in vaccination, and the lack of antiviral treatment. Aid projects are urgently needed, however impeded by North Korea's political and economical situation and isolation. The feasibility of a joint North Korean and German humanitarian hepatitis B prevention program was assessed. Part 1: Hepatitis B vaccination catch-up campaign. Part 2: Implementation of endoscopic ligation of esophageal varices (EVL) by trainings in Germany and North Korea. By vaccinating 7 million children between 2010 and 2012, the hepatitis B vaccination gap was closed. Coverage of 99.23% was reached. A total of 11 hepatitis B-induced liver cirrhosis patients (mean age 41.1 yr) with severe esophageal varices and previous bleedings were successfully treated by EVL without major complications. A clinical standard operating procedure, a feedback system and a follow-up plan were developed. The bi-modal preventive strategy was implemented successfully. Parts of the project can serve as an example for other low-income countries, however its general transferability is limited due to the special circumstances in North Korea.
Human cysticercosis caused by Taenia crassiceps tapeworm larvae involves the muscles and subcutis mostly in immunocompromised patients and the eye in immunocompetent persons. We report a successfully treated cerebellar infection in an immunocompetent woman. We developed serologic tests, and the parasite was identified by histologic examination and 12s rDNA PCR and sequencing.
Background: In the heart, cytoplasmic actin networks are thought to have important roles in mechanical support, myofibrillogenesis, and ion channel function. However, subcellular localization of cytoplasmic actin isoforms and proteins involved in the modulation of the cytoplasmic actin networks are elusive. Mena and VASP are important regulators of actin dynamics. Due to the lethal phenotype of mice with combined deficiency in Mena and VASP, however, distinct cardiac roles of the proteins remain speculative. In the present study, we analyzed the physiological functions of Mena and VASP in the heart and also investigated the role of the proteins in the organization of cytoplasmic actin networks.
Results: We generated a mouse model, which simultaneously lacks Mena and VASP in the heart. Mena/VASP double-deficiency induced dilated cardiomyopathy and conduction abnormalities. In wild-type mice, Mena and VASP specifically interacted with a distinct αII-Spectrin splice variant (SH3i), which is in cardiomyocytes exclusively localized at Z- and intercalated discs. At Z- and intercalated discs, Mena and β-actin localized to the edges of the sarcomeres, where the thin filaments are anchored. In Mena/VASP double-deficient mice, β-actin networks were disrupted and the integrity of Z- and intercalated discs was markedly impaired.
Conclusions: Together, our data suggest that Mena, VASP, and αII-Spectrin assemble cardiac multi-protein complexes, which regulate cytoplasmic actin networks. Conversely, Mena/VASP deficiency results in disrupted β-actin assembly, Z- and intercalated disc malformation, and induces dilated cardiomyopathy and conduction abnormalities.
Background: Acute schistosomiasis constitutes a rare but serious condition in individuals experiencing their first prepatent Schistosoma infection. To circumvent costly and time-consuming diagnostics, an early and rapid diagnosis is required. So far, classic diagnostic tools such as parasite microscopy or serology lack considerable sensitivity at this early stage of Schistosoma infection. To validate the use of a blood based real-time polymerase chain reaction (PCR) test for the detection of Schistosoma DNA in patients with acute schistosomiasis who acquired their infection in various endemic regions we conducted a European-wide prospective study in 11 centres specialized in travel medicine and tropical medicine.
Methods: Patients with a history of recent travelling to schistosomiasis endemic regions and freshwater contacts, an episode of fever (body temperature >= 38.5 degrees C) and an absolute or relative eosinophil count of >= 700/mu l or 10%, were eligible for participation. PCR testing with DNA extracted from serum was compared with results from serology and microscopy. Results: Of the 38 patients with acute schistosomiasis included into the study, PCR detected Schistosoma DNA in 35 patients at initial presentation (sensitivity 92%). In contrast, sensitivity of serology (enzyme immunoassay and/or immunofluorescence assay) or parasite microscopy was only 70% and 24%, respectively.
Conclusion: For the early diagnosis of acute schistosomiasis, real-time PCR for the detection of schistosoma DNA in serum is more sensitive than classic diagnostic tools such as serology or microscopy, irrespective of the region of infection. Generalization of the results to all Schistosoma species may be difficult as in the study presented here only eggs of S. mansoni were detected by microscopy. A minimum amount of two millilitre of serum is required for sufficient diagnostic accuracy.

Background
Chronic thromboembolic pulmonary hypertension (CTEPH) is a long-term complication following an acute pulmonary embolism (PE). It is frequently diagnosed at advanced stages which is concerning as delayed treatment has important implications for favourable clinical outcome. Performing a follow-up examination of patients diagnosed with acute PE regardless of persisting symptoms and using all available technical procedures would be both cost-intensive and possibly ineffective. Focusing diagnostic procedures therefore on only symptomatic patients may be a practical approach for detecting relevant CTEPH.
This study aimed to evaluate if a follow-up program for patients with acute PE based on telephone monitoring of symptoms and further examination of only symptomatic patients could detect CTEPH. In addition, we investigated the role of cardiopulmonary exercise testing (CPET) as a diagnostic tool.
Methods
In a prospective cohort study all consecutive patients with newly diagnosed PE (n=170, 76 males, 94 females within 26 months) were recruited according to the inclusion and exclusion criteria. Patients were contacted via telephone and asked to answer standardized questions relating to symptoms. At the time of the final analysis 130 patients had been contacted. Symptomatic patients underwent a structured evaluation with echocardiography, CPET and complete work-up for CTEPH.
Results
37.7%, 25.5% and 29.3% of the patients reported symptoms after three, six, and twelve months respectively. Subsequent clinical evaluation of these symptomatic patients saw 20.4%, 11.5% and 18.8% of patients at the respective three, six and twelve months time points having an echocardiography suggesting pulmonary hypertension (PH). CTEPH with pathological imaging and a mean pulmonary artery pressure (mPAP) ≥ 25 mm Hg at rest was confirmed in eight subjects. Three subjects with mismatch perfusion defects showed an exercise induced increase of PAP without increasing pulmonary artery occlusion pressure (PAOP). Two subjects with pulmonary hypertension at rest and one with an exercise induced increase of mPAP with normal PAOP showed perfusion defects without echocardiographic signs of PH but a suspicious CPET.
Conclusion
A follow-up program based on telephone monitoring of symptoms and further structured evaluation of symptomatic subjects can detect patients with CTEPH. CPET may serve as a complementary diagnostic tool.
Alzheimer’s disease (AD) is the most prevalent neurodegenerative disease of the brain, which is characterized by a progressive loss of memory and spatial orientation. Only less than 5-10% of AD sufferers are familial cases due to genetic mutations in the amyloid precursor protein (APP) gene or presenilin (PS) 1 and 2 genes. The cause of sporadic AD (sAD) which covers > 95% of AD patients is still unknown. Current research found interactions between aging, diabetes and cognitive decline including dementia in general and in AD in particular. Disturbances of brain glucose uptake, glucose tolerance and utilization and impairment of the insulin/insulin receptor (IR) signaling cascade are thought to be key targets for the development of sAD.
In the brain of AD patients, neural plasticity is impaired indicated by synaptic and neuronal loss. Adult neurogenesis (AN), the generation of functional neurons in the adult brain, may be able to restore neurological function deficits through the integration of newborn neurons into existing neural networks. The dentate gyrus of the hippocampus is one out of few brain regions where life-long AN exists. However, there is a big controversy in literature regarding the involvement of AN in AD pathology. Most animal studies used transgenic mice based on the Amyloid ß (Aß) hypothesis which primarily act as models for the familial form of AD. Findings from human post mortem AN studies were also inconstistent. In this thesis, we focused on the possible involvement of AN in the pathogenesis of the sporadic form of AD. Streptozotocin intracerebroventricularily (STZ icv) treated rats, which develop an insulin-resistant brain state and learning and memory deficits preceding Aß pathology act as an appropriate animal model for sAD. We used STZ treatment for both parts of my work, for the in vivo and in vitro study.
In the first part of my thesis, my coworkers and I investigated STZ icv treatment effects on different stages of AN in an in vivo approach. Even if STZ icv treatment does not seem to considerably influence stem cell proliferation over a short-term (1 month after STZ icv treatment) as well as in a long-term (3 months after STZ icv treatment) period, it results in significantly less immature and newborn mature neurons 3 months after STZ icv treatment. This reduction detected after 3 months was specific for the septal hippocampus, discussed to be important for spatial learning. Subsequently we performed co-localization studies with antibodies detecting BrdU (applied appr. 27 days before sacrifice) and cell-type specific markers such as NeuN, and GFAP, we found that STZ treatment does not affect the differentiation fate of newly generated cells. Phenotype analysis of BrdU-positive cells in the hilus and molecular layer revealed that some of the BrdU-positive cells are newborn oligodendrocytes but not newborn microglia.
In the second part of my thesis I worked with cultured neural stem cells (NSCs) isolated from the adult rat hippocampus to reveal STZ effects on the proliferation of of NSCs, and on the survival and differentiation of their progeny. Furthermore, this in vitro approach enabled me to study cellular mechanisms underlying the observed impaired neurogenesis in the hippocampus of STZ-treated rats. In contrast to our findings of the STZ icv in vivo study we revealed that STZ supplied with the cell culture medium inhibits the proliferation of NSCs in a dose-dependent and time-dependent manner. Moreover, performing immunofluorescence studies with antibodies detecting cell-type specific markers after triggering NSCs to differentiate, we could show that STZ treatment affects the number of newly generated neurons but not of astrocytes. Analyzing newborn cells starting to differentiate and migrate I was able to demonstrate that STZ has no effect on the migration of newborn cells. Trying to reveal cellular mechanisms underlying the negative influence of STZ on hippocampal AN, we performed qRT-PCR and immunofluorescence staining and thus could show that in NSCs the expression of glucose transporter (GLUT)3 mRNA as well as IR and GLUT3 protein levels are reduced after STZ treatment. Therefore, the inhibition of the proliferation of NSCs may be (at least partially) caused by these two molecules. Interestingly, the effect of STZ on differentiating cells was shown to be different, as IR protein expression was not significantly changed but GLUT3 protein levels were decreased in consequence of STZ treatment.
In summary, this project delivered further insights into the interrelation between AN the sporadic form of sAD and thus provides a basis of new therapeutic approaches in sAD treatment through intervening AN. Discrepancies between the results of the two parts of my thesis, the in vivo and in vitro part, were certainly caused to a certain extent by the missing microenvironment in the in vitro approach with cultured NSCs. Future studies e.g. using co-culture systems could at least minimize the effect of a missing natural microenvironment of cultured NSCs, so that the use of an in vitro approach for the investigation of STZ treatment underlying cellular mechanisms can be improved.
Fragestellung
Die Prognose eines akuten Hirninfarktes bei Verschluss einer proximalen Hirnarterie ist trotz der intravenösen Thrombolyse mit rtPA ungünstig. Kann die kombinierte pharmaco-mechanische Rekanalisation von proximalen Gefäßverschlüssen bei akutem Hirninfarkt zu einer Verbesserung des klinischen Ergebnisses führen?
Methoden
Wir analysierten retrospektiv 66 konsekutiv aufgenommene Patienten (36m, 30w; mittleres Alter 61 Jahre (23-86 Jahre), die von 2010 bis 2012 kombiniert pharmako-mechanisch intra-arteriell behandelt wurden. 32 Patienten wiesen einen kombinierten ACI-/M1-Verschluss, 23 einen M1-Verschluss und 11 eine Basilaristhrombose auf. Mittlerer NIHSS lag bei 23. 57 Patienten erhielten eine kombinierte pharmaco-mechanische Therapie, 3 Patienten wurden lediglich pharmakologisch und 6 Patienten rein mechanisch rekanalisiert. Rekanalisierung bei 35 Patienten mit einem Stent-Retriever (32 Patienten mit pREset, 3 Patienten mit SOLITAIRE) erfolgt. Bei 46 Patienten wurde rtPA und bei 32 Patienten Tirofiban als Bridging Verfahren eingesetzt. Eine Stentanlage erfolgte in 28,78% der Fälle.
Ergebnisse
Die erzielten Rekanalisationsraten lagen bei 89,4% bei einer mittleren Dauer der Intervention von 96 Minuten (53,03% unter 90 Min.). Ein günstiges klinisches Ergebnis nach mRS (mRS 0-2) wurde bei 48% der Patienten erreicht. Die Rate an symptomatischen intrazerebralen Blutungen lag bei 4,55%. Die Mortalität war 19,7%. Die multivariate Regressionsanalyse ergab als modifizierbare Prediktoren für ein günstiges Outcome die Dauer bis zur Rekanalisation und die Gabe von rtPA.
Schlussfolgerungen
Die kombinierte endovaskuläre pharmako-mechanische Therapie kann die Mortalität und Morbidität von Schlaganfallpatienten mit Verschlüssen einer proximalen Hirnarterie reduzieren.
In May 2014, six patients presented in Germany with a Sarcocystis-associated febrile myositis syndrome after returning from Tioman Island, Malaysia. During two earlier waves of infections, in 2011 and 2012, about 100 travellers returning to various European countries from the island were affected. While the first two waves were associated with travel to Tioman Island mostly during the summer months, this current series of infections is associated with travel in early spring, possibly indicating an upcoming new epidemic.
Pocket-Proteine und E2F-Transkriptionsfaktoren regulieren die Expression von Zellzyklus-assoziierten Genen und spielen eine zentrale Rolle bei der Koordination der Zellteilung, Differenzierung und Apoptose. Störungen dieser Signalwege tragen zur Entstehung zahlreicher Tumorentitäten beim Menschen bei. Trotz der intensiven Untersuchung der Zellzyklusregulation sind viele Details noch unverstanden.
Der LIN-Komplex (LINC / DREAM) ist ein kürzlich entdeckter humaner Multiprotein-komplex, welcher dynamisch mit Pocket-Proteinen und E2F-Transkriptionsfaktoren interagiert. Eine essentielle Komponente des LIN-Komplexes ist das LIN9-Protein. Um die Funktion dieses Proteins bei der Zellzyklusregulation und Tumorentstehung genauer untersuchen zu können, wurde in unserer Arbeitsgruppe ein konditionelles Lin9-Knockout-Mausmodell etabliert.
Primäres Ziel der Arbeit war es, den Phänotyp embryonaler Fibroblasten (MEFs) aus diesen Mäusen zu charakterisieren. Bereits kurz nach Inaktivierung von Lin9 konnte ein stark verlangsamtes Zellwachstums beobachtet werden. In Lin9-depletierten MEFs wurden multiple mitotische Defekte detektiert, die u. a. strukturelle Auffälligkeiten des Spindelapparates, aberrante Zellkerne, Störungen der Chromosomensegregation sowie zytokinetische Defekte umfassen und in einer dramatischen Zunahme polyploider und aneuploider Zellen resultieren. Im Langzeitverlauf führen diese erheblichen Aberrationen zu einer vorzeitigen zellulären Seneszenz. Wird diese durch das Large T-Protoonkogen durchbrochen, können sich MEFs an den Verlust von Lin9 adaptieren, zeigen dann jedoch eine hochgradige genomische Instabilität und Substrat-unabhängiges Wachstum im Weichagar als Zeichen onkogener Transformation.
Im zweiten Abschnitt der vorliegenden Arbeit wurde die Genexpression in Lin9-defizienten MEFs mittels quantitativer Real Time-PCR untersucht um zu klären, ob die beschriebenen Defekte auf Veränderungen der transkriptionellen Aktivität zurück-zuführen sind. Dabei wurde eine erhebliche Reduktion der Expressionslevel mitotischer Gene nach Verlust von Lin9 beobachtet. Des Weiteren wurden zur Klärung der zu Grunde liegenden molekularen Mechanismen Chromatin-Immunpräzipitations-Experimente (ChIP) durchgeführt. Im Vergleich zu Kontrollzellen wurden dabei in Lin9-defizienten Zellen signifikante epigenetische Veränderungen bezüglich aktivierender Histon-Modifikationen an den Promotoren mitotischer Lin9-Zielgene festgestellt.
Im letzten Abschnitt der Arbeit sollten die Auswirkungen des heterozygoten Verlustes von Lin9 analysiert werden. Dabei zeigte sich, dass Lin9-haploinsuffiziente Zellen normal proliferieren, obwohl die Expression verschiedener G2/M-Gene leicht vermindert war. Es wurde jedoch eine Schwächung des mitotischen Spindelkontrollpunktes und in der Folge über mehrere Zellgenerationen eine Zunahme polyploider Zellen beobachtet. Mit Weichagar-Assays konnte gezeigt werden, dass bereits der heterozygote Verlust des Lin9-Gens zur onkogenen Transformation beiträgt.
Zusammengenommen dokumentieren diese Studien, dass LIN9 eine entscheidende Bedeutung bei der Regulation von Zellzyklus-assoziierten Genen spielt und sowohl einen essentiellen Faktor für die Zellproliferation darstellt als auch durch die Gewährleistung genomischer Stabilität tumorsuppressive Eigenschaften aufweist.
Im Rahmen der Abschlussuntersuchung der Fall-Kontroll-Studie (September 2005- Oktober 2007) des Missionsärztlichen Instituts Würzburg in Kaduna, Nigeria, wurde die spezifische Symptomatik, die Blutwerte sowie die motorische Leistungsfähigkeit bei 124 Kindern mit Rachitis und 87 gesunden Kontrollen im Alter von 1 bis 18 Jahren analysiert und mit den Eingangs- und Verlaufswerten verglichen. Dabei wurden die Dimensionen der subjektiven und objektiven Einschränkungen des Bewegungsapparates, der motorischen Fähigkeiten Koordination, Kraft, Ausdauer und Beweglichkeit sowie der krankheitsspezifischen Serumwerte Calcium, PTH und Vitamin D gemessen und nach Alter, Geschlecht, klinisch-orthopädischen Untersuchungsergebnissen und durchgeführter Intervention analysiert. Die 2005 begonnene Substitution mit Calciumcarbonat über 3 Monate führte bei 54 Kindern zu einer Angleichung an die erhobenen Kontrollwerte. In Relation zu vergleichbaren Interventionsstudien kam es zu einer reduzierten Besserung. Begann die Therapie vor der motorischen Entwicklungsphase vor dem 7. Lebensjahr, so konnten Leistungsminderungen kompensiert werden. Die erhobenen Werte der 58 Teilnehmer der zweiten Interventionsgruppe mit Calciumlaktat sind zwischen den Studienergebnissen der aktiven und der therapierten Rachitis einzuordnen. Erstmals wurden die Eingangs- und Ausgangswerte von 12 symptomatischen Teilnehmer ohne wahrgenommener Therapie dokumentiert, mit dem Ergebnis einer vergleichbaren Besserung der klinischen und serologischen Werte wie die therapierten Teilnehmer. Die Ergebnisse bestätigen erstmals anhand eines objektiven Testverfahrens die Annahme, dass zwischen dem Ausprägungsgrad der Rachitis und der motorischen Leistungsfähigkeit ein Zusammenhang besteht. Die aufgezeigten Ergebnisse
verdeutlichen, dass eine Calcium-Mangel-Rachitis erfolgreich mit einer alleinige Substitution von Calcium therapiert werden kann, besonders vor dem 7. Lebensjahr. Dabei gilt neben einer ausreichenden Substitution an Calcium auch eine adäquate Compliance sicherzustellen. Mit der Erhebung der klinischen, motorischen und serologischen Werte wurde eine Datenbasis geschaffen, anhand derer es zukünftig möglich sein wird, Aussagen über den Verlauf und die adäquate Therapie von Kindern mit Rachitis in Nigeria zu treffen.
Bacteria organize many membrane-related signaling processes in functional microdomains that are structurally and functionally similar to the lipid rafts of eukaryotic cells. An important structural component of these microdomains is the protein flotillin, which seems to act as a chaperone in recruiting other proteins to lipid rafts to facilitate their interaction. In eukaryotic cells, the occurrence of severe diseases is often observed in combination with an overproduction of flotillin, but a functional link between these two phenomena is yet to be demonstrated. In this work, we used the bacterial model Bacillus subtilis as a tractable system to study the physiological alterations that occur in cells that overproduce flotillin. We discovered that an excess of flotillin altered specific signal transduction pathways that are associated with the membrane microdomains of bacteria. As a consequence of this, we detected significant defects in cell division and cell differentiation. These physiological alterations were in part caused by an unusual stabilization of the raft-associated protease FtsH. This report opens the possibility of using bacteria as a working model to better understand fundamental questions related to the functionality of lipid rafts.
IMPORTANCE The identification of signaling platforms in the membrane of bacteria that are functionally and structurally equivalent to eukaryotic lipid rafts reveals a level of sophistication in signal transduction and membrane organization unexpected in bacteria. It opens new and promising venues to address intricate questions related to the functionality of lipid rafts by using bacteria as a more tractable system. This is the first report that uses bacteria as a working model to investigate a fundamental question that was previously raised while studying the role of eukaryotic lipid rafts. It also provides evidence of the critical role of these signaling platforms in orchestrating diverse physiological processes in prokaryotic cells.
Hintergrund: Dank der modernen Medizin werden Krebserkrankungen immer häufiger und früher entdeckt. Immer mehr onkologische Patienten leben über einen langen Zeitraum mit ihrer Krebserkrankung, sodass Krebs mehr und mehr zur chronischen Erkrankung wird. Von diesen Patienten nimmt ein sehr hoher Anteil Alternativmedizin in Anspruch, oft Wissen der behandelnden Ärzte. Dabei sind sich viele Patienten der Tatsache nicht bewusst, dass zahlreiche vermeintlich natürliche alternative Heilmittel durchaus relevante Neben- und Wechselwirkungen haben können. Ziel der vorliegenden Arbeit war es, aktuelle Prävalenzwerte für die Inanspruchnahme von Alternativmedizin durch Krebspatienten zu erfassen und Patientenmerkmale zu finden, die in Zusammenhang mit der Nutzung alternativmedizinischer Methoden stehen.
Methoden: Es wurden Patientendaten verwendet, die im Rahmen der multizentrischen, von der Deutschen Krebshilfe geförderten Studie „Prävalenz psychosozialer Belastungen, psychischer Störungen und ihr Behandlungsbedarf bei Tumorpatienten“ im Studienzentrum Würzburg erhoben wurden. Neben Angaben zur Nutzung von Alternativmedizin wurden mit Hilfe eines vom Patienten auszufüllenden Fragebogenheftes verschiedene soziodemographische und psychosoziale Variablen erfasst und durch medizinische Daten aus Krankenakten ergänzt. Mögliche Prädiktoren wurden mittels stufenweiser logistischer Regression ermittelt.
Ergebnisse: Von den 770 Würzburger Studienteilnehmern konnten 699 im Alter zwischen 21 und 75 Jahren für die vorliegende Arbeit zur Analyse herangezogen werden. Insgesamt nutzte etwa jeder dritte Patient (33,6%) Alternativmedizin im Zusammen-hang mit der Krebserkrankung, was sich gut mit Daten aus der aktuellen Forschung deckt. 71,5% der Nutzer waren weiblich. Am häufigsten wurden Mittel aus den Kategorien „Vitamine, Mineralien, Spurenelemente“ (24% aller Patienten), „Homöopathie“ (11,6%) und „Misteltherapie“ (6,4%) angewandt. Als Bezugsquelle der alternativmedizinischen Mittel diente in der Mehrzahl der Fälle (62,5%) ein Arzt. Alle alternativmedizinischen Angebote wurden von den Anwendern hoch in ihrer Nützlichkeit bewertet. Darüber hinaus wurden soziodemographische, medizinische und psychosoziale Variablen auf einen Zusammenhang mit der Inanspruchnahme von Alternativmedizin untersucht. Entsprechend dem aktuellen Forschungsstand erwiesen sich dabei weibliches Geschlecht und höherer Bildungsstand als unabhängige Prädiktoren für die Nutzung von Alternativmedizin. Ein Zusammenhang mit dem Alter konnte hingegen nicht bestätigt werden. Im Hinblick auf krankheits- und therapieassoziierte Variablen waren der Abschluss einer Strahlentherapie und die Angabe von krebsbedingten Schmerzen von unabhängigem prädiktiven Wert, während für den Abschluss einer Chemotherapie und die Tumorlokalisation lediglich bivariate Zusammenhänge bestanden. Ein solcher Zusammenhang mit der Inanspruchnahme von Alternativmedizin konnte auch für die psychosozialen Variablen höhere psychische Belastung und geringere Lebensqualität nachgewiesen werden, die sich jedoch in der logistischen Regression nicht als unabhängige Prädiktoren bestätigten. Von den psychosozialen Variablen stellte der Wunsch nach aktiver Partizipation am Therapieprozess den einzigen unabhängigen Prädiktor für die Nutzung alternativmedizinischer Angebote dar.
Schlussfolgerungen: Ein Drittel aller Krebspatienten wendet Alternativmedizin im Zusammenhang mit der Krebserkrankung an. Daher sollte die medizinische Wissenschaft es als wichtige Aufgabe sehen, fundierte Daten zu Nutzen und Sicherheit alternativmedizinischer Mittel bereitzustellen, um dieses weite Feld nicht unseriösen Anbietern zu überlassen. Auch der rein konventionell tätige Arzt sollte sich der großen Beliebtheit von Alternativ-medizin bei bestimmten Patientengruppen bewusst sein und seinen Patienten Auskunft zu alternativmedizinischen Angeboten geben können.
Murine embryonale Stammzellen (ES-Zellen) stellen mit ihrem Selbsterneuerungs- und Differenzierungspotenzial einen einzigartigen Zelltyp für die Grundlagenforschung und angewandte Wissenschaften dar. Auf Grund ihrer Fähigkeit, in vitro die embryonale Entwicklung eines Organismus nachzuahmen, sind sie für die Untersuchung der Zell-Differenzierung, wie z.B. der embryonalen Hämatopoese geeignet. Während der ES-Zell-Selbsterneuerung und -Differenzierung spielen epigenetischen Modifikationen, unter anderem Histon-Methylierungen, eine wichtige Rolle. Transkriptionell aktivierende (H3K4me2/3, di- bzw. trimethyliertes Lysin 4 an Histon 3) und reprimierende (H3K27me2/3; di- bzw. trimethyliertes Lysin 27 an Histon 3) Histon-Methylierungs-Muster und die epigenetische Gen-Regulierung werden unter anderem durch die entgegenwirkenden PcG- und MLL-Protein-Komplexe koordiniert. Die H3K27me2/3-spezifische Demethylase UTX/KDM6A ist eine Komponente des MLL-Komplexes und somit an aktivierenden Gen-Regulationsmechanismen beteiligt. Im Rahmen dieser Arbeit war es mein Ziel zu untersuchen, inwieweit UTX für die Aufrechterhaltung der ES-Zell-Pluripotenz und für die ES-Zell-Differenzierung, insbesondere die hämatopoetische Differenzierung, von Bedeutung ist. Meine Daten zeigten, dass UTX in undifferenzierten ES-Zellen, während der ES-Zell-Differenzierung und in adulten Geweben ubiquitär exprimiert ist. Um Aufschluss über die UTX-Funktion zu bekommen, wurde UTX in ES-Zellen mittels RNA-Interferenz und Gene-Targeting gezielt ablatiert. Genexpressions-Analysen zeigten, dass die Expression von Pluripotenzgenen, genauso wie die Zellproliferation und die Verteilung der Zellzyklus-Phasen in ES-Zellen durch den Verlust von UTX unbeeinflusst blieben, während globale H3K4me3- sowie H3K27me3-Level reduziert waren. Während der ES-Zell-Differenzierung konnte ich eine verminderte Induktion der mesodermalen und hämatopoetischen Marker Flk1, Brachyury, Runx1 und Gata1 beobachten. Zudem war die Expression von UTY, dem auf dem Y-Chromosom kodierten UTX-Homolog, in ES-Zellen und während der Differenzierung runterreguliert, was auf eine Regulierung durch UTX schließen lässt. Des Weiteren zeigten UTX-Knockdown und –Knockout-Zellen in funktionellen hämatopoetischen in vitro Assays eine verminderte Fähigkeit, Blast-Kolonien und hämatopoetische Vorläuferzellen zu generieren. Interessanterweise zeigten ChIP-Analysen in differenzierenden wt und UTX-Knockout-EBs unveränderte H3K27me3-Level an Promotoren der hämatopoetischen Gene, was auf eine Demethylase-unabhängige Funktion von UTX während der frühen Hämatopoese hindeutet. Um die Funktion von UTX während der Entwicklung in vivo, insbesondere während der embryonalen Hämatopoese, untersuchen zu können, habe ich eine konditionelle UTX-Knockout-Maus hergestellt, die für eine gezielte UTX-Deletion im hämatopoetischen System verwendet wird. Zusammenfassend zeigen meine Daten, dass UTX für die ES-Zell-Proliferation und –Pluripotenz unbedeutend ist und die Reduzierung der H3K27-Trimethylierung auch bei fehlendem UTX weiterhin herbeigeführt werden kann. Im Gegensatz dazu übernimmt UTX eine entscheidende Rolle während der mesodermalen und hämatopoetischen ES-Zell-Differenzierung, vermutlich über eine Histon-Demethylase-unabhängige Funktion.
Cardiac healing after myocardial infarction (MI) represents the cardinal prerequisite for proper replacement of the irreversibly injured myocardium. In contrast to innate immunity, the functional role of adaptive immunity in postinfarction healing has not been systematically addressed. The present study focused on the influence of CD4+ T lymphocytes on wound healing and cardiac remodeling after experimental myocardial infarction in mice. Both conventional and Foxp3+ regulatory CD4+ T cells (Treg cells) became activated in heart draining lymph nodes after MI and accumulated in the infarcted myocardium. T cell activation was strictly antigen-dependant as T cell receptor-transgenic OT-II mice in which CD4+ T cells exhibit a highly limited T cell
receptor repertoire did not expand in heart-draining lymph nodes post-MI. Both OT-II and major histocompatibility complex class II-deficient mice lacking a CD4+ T cell compartment showed a fatal clinical postinfarction outcome characterized by disturbed scar tissue construction that resulted in impaired survival due to a prevalence of left-ventricular ruptures. To assess the contribution of anti-inflammatory Treg cells on wound healing after MI, the Treg cell compartment was depleted using DEREG mice that specifically express the human diphtheria toxin receptor in Foxp3-positive cells, resulting in Treg cell ablation after diphtheria toxin administration. In a parallel line of experiments, a second model of anti-CD25 antibody-mediated Treg cell immuno-depletion was used. Treg cell ablation prior to MI resulted in adverse postinfarction left-ventricular dilatation associated with cardiac deterioration. Mechanistically, Treg cell depletion resulted in an increased recruitment of pro-inflammatory neutrophils and Ly-6Chigh monocytes into the healing myocardium. Furthermore, Treg cell-ablated mice exhibited an adverse activation of conventional non-regulatory CD4+ and CD8+ T cells that
showed a reinforced infiltration into the infarct zone. Increased synthesis of TNFα and IFNγ by conventional CD4+ and CD8+ T cells in hearts of Treg cell-depleted mice provoked an M1-like macrophage polarization characterized by heightened expression of healing-compromising induced NO synthase, in line with a reduced synthesis of healing-promoting transglutaminase factor XIII (FXIII), osteopontin (OPN) and transforming growth factor beta 1 (TGFβ1).
Therapeutic Treg cell activation by a superagonistic anti-CD28 monoclonal antibody stimulated Treg cell accumulation in the infarct zone and led to an increased expression of mediators inducing an M2-like macrophage polarization state, i.e. interleukin-10, interleukin-13 and TGFβ1. M2-like macrophage differentiation in the healing infarct was associated with heightened expression of scar-forming procollagens as well as scar-stabilizing FXIII and OPN, resulting in improved survival due to a reduced incidence of left-ventricular ruptures. Therapeutic Treg cell activation and the induction of a beneficial M2-like macrophage polarization was further achieved by employing a treatment modality of high clinical potential, i.e. by therapeutic administration of IL-2/ anti-IL-2 monoclonal antibody complexes. The findings of the present study suggest that therapeutic Treg cell activation and the resulting improvement of healing may represent a suitable strategy to attenuate adverse infarct expansion, left-ventricular remodeling, or infarct ruptures in patients with MI.
Diese Arbeit soll mit Hilfe einer prospektiven, multizentrischen Doppelblind-Studie zeigen, dass durch den Einsatz von Pioglitazon bei Patienten mit Diabetes mellitus Typ II und dialysepflichtiger Niereninsuffizienz, die tägliche Insulindosis signifikant reduziert werden kann. Durch das orale Antidiabetikum soll gezeigt werden, dass die Patienten sich deutlich weniger Insulin injizieren müssen, eine bessere Einstellung der diabetischen Stoffwechsellage erreicht wird und somit die Komplikationsrate sowie die Wahrscheinlichkeit einer Nebenwirkung durch die Insulintherapie sinkt. Des Weiteren sollte überprüft werden, ob bekannte pleiotrope Effekte von Pioglitazon ebenfalls beobachtet werden können.
Viele unterschiedliche antidiabetisch wirkende Parameter im Blut wurden mittels laborchemischer Untersuchung über einen Zeitraum von einem halben Jahr miteinander verglichen, um die Wirkung von Pioglitazon auf den Diabetes mellitus zu bestätigen. Zusätzlich wurde mit Hilfe von bestimmten Laborparametern versucht ein möglichst genaues kardiovaskuläres Profil der einzelnen Probanden unter Therapie zu ermitteln, um mögliche pleiotrope Effekte des Medikaments nachzuweisen.
