Telemedicine uses telecommunication and information technology to provide health care services over spatial distances. In the upcoming demographic changes towards an older average population age, especially rural areas suffer from a decreasing doctor to patient ratio as well as a limited amount of available medical specialists in acceptable distance. These areas could benefit the most from telemedicine applications as they are known to improve access to medical services, medical expertise and can also help to mitigate critical or emergency situations. Although the possibilities of telemedicine applications exist in the entire range of healthcare, current systems focus on one specific disease while using dedicated hardware to connect the patient with the supervising telemedicine center.
This thesis describes the development of a telemedical system which follows a new generic design approach. This bridges the gap of existing approaches that only tackle one specific application. The proposed system on the contrary aims at supporting as many diseases and use cases as possible by taking all the stakeholders into account at the same time. To address the usability and acceptance of the system it is designed to use standardized hardware like commercial medical sensors and smartphones for collecting medical data of the patients and transmitting them to the telemedical center. The smartphone can also act as interface to the patient for health questionnaires or feedback.
The system can handle the collection and transport of medical data, analysis and visualization of the data as well as providing a real time communication with video and audio between the users.
On top of the generic telemedical framework the issue of scalability is addressed by integrating a rule-based analysis tool for the medical data. Rules can be easily created by medical personnel via a visual editor and can be personalized for each patient. The rule-based analysis tool is extended by multiple options for visualization of the data, mechanisms to handle complex rules and options for performing actions like raising alarms or sending automated messages.
It is sometimes hard for the medical experts to formulate their knowledge into rules and there may be information in the medical data that is not yet known. This is why a machine learning module was integrated into the system. It uses the incoming medical data of the patients to learn new rules that are then presented to the medical personnel for inspection. This is in line with European legislation where the human still needs to be in charge of such decisions.
Overall, we were able to show the benefit of the generic approach by evaluating it in three completely different medical use cases derived from specific application needs: monitoring of COPD (chronic obstructive pulmonary disease) patients, support of patients performing dialysis at home and councils of intensive-care experts. In addition the system was used for a non-medical use case: monitoring and optimization of industrial machines and robots. In all of the mentioned cases, we were able to prove the robustness of the generic approach with real users of the corresponding domain. This is why we can propose this approach for future development of telemedical systems.
The quest for finding a unifying theory for both quantum theory and gravity lies at the heart of much of the research in high energy physics. Although recent years have witnessed spectacular experimental confirmation of our expectations from Quantum Field Theory and General Relativity, the question of unification remains as a major open problem. In this context, the perturbative aspects of quantum black holes represent arguably the best of our knowledge of how to proceed in this pursue.
In this thesis we investigate certain aspects of quantum gravity in 2 + 1 dimensional anti-de Sitter space (AdS3), and its connection to Conformal field theories in 1 + 1 dimensions (CFT2), via the AdS/CFT correspondence.
We study the thermodynamics properties of higher spin black holes. By focusing on the spin-4 case, we show that black holes carrying higher spin charges display a rich phase diagram in the grand canonical ensemble, including phase transitions of the Hawking-Page type, first order inter-black hole transitions, and a second order critical point.
We investigate recent proposals on the connection between bulk codimension-1 volumes and computational complexity in the CFT. Using Tensor Networks we provide concrete evidence of why these bulk volumes are related to the number of gates in a quantum circuit, and exhibit their topological properties. We provide a novel formula to compute this complexity directly in terms of entanglement entropies, using techniques from Kinematic space.
We then move in a slightly different direction, and study the quantum properties of black holes via de Functional Renormalisation Group prescription coming from Asymptotic safety. We avoid the arbitrary scale setting by restricting to a narrower window in parameter space, where only Newton’s coupling and the cosmological constant are allowed to vary. By one assumption on the properties of Newton’s coupling, we find black hole solutions explicitly. We explore their thermodynamical properties, and discover that very large black holes exhibit very unusual features.
Non-steroidal antiinflammatory drugs are most commonly used for inflammatory and postoperative pain. But they lack effectiveness and specificity, leading to severe side effects, like gastric ulcers, asthma and severe bleeding. Oxidized 1-palmitoyl-2-arachinidonoyl-sn-glycero-3-phosphocholine (OxPAPC) plays an important role in inflammatory pain. PAPC is a common phosphatidylcholine of membranes, which can be oxidized by reactive oxygen species. In preliminary experiments, our group found that local injection of OxPAPC in rat paws induces hyperalgesia.
In this study we examined the effect of OxPAPC on transient receptor potential A1 (TRPA1), an ion channel expressed in C-fiber neurons. Furthermore, we investigated if intracellular cysteine residues of TRPA1 were necessary for agonist-channel-interactions and if a subsequent TRPA1 activation could be prevented by OxPAPC scavengers.
To answer these questions, we performed calcium imaging using HEK-293 cells stably expressing hTRPA1, or transiently expressing the triple mutant channel hTRPA1-3C and naïve DRG neurons. Cells were incubated with the ratiometric, fluorescent dye Fura-2/AM and stimulated with OxPAPC. The change of light emission after excitation with 340 and 380 nm wavelengths allowed conclusions regarding changes of intracellular calcium concentrations after TRPA1 activation.
In our investigation we proved evidence that OxPAPC activates TRPA1, which caused a flow of calcium ions into the cytoplasm. The TRPA1-specific channel blocker HC-030031 eliminated this agonist-induced response. TRPA1-3C was not completely sensitive to OxPAPC. The peptide D-4F and the monoclonal antibody E06 neutralized OxPAPC-induced TRPA1 activation.
In this work, the importance of OxPAPC as a key mediator of inflammatory pain and as a promising target for drug design is highlighted. Our results indicate that TRPA1 activation by OxPAPC involves cysteine-dependent mechanisms, but there are other, cysteine-independent activation mechanisms as well. Potential pharmaceuticals for the treatment of inflammatory pain are D-4F and E06, whose efficiency has recently been confirmed in the animal model by our research group.
Charcot-Marie-Tooth Neuropathien sind die häufigsten hereditären Erkrankungen des peripheren Nervensystems und dennoch bis heute nicht therapierbar. Die Lebensqualität der Patienten ist durch motorische und sensorische Defizite der Extremitäten häufig stark eingeschränkt. Ursache können unter anderem Mutationen in Schwann-Zellen sein, die zu dem typischen Bild von Demyelinisierung und axonalem Schaden führen. In den letzten Jahren konnte in Mausmodellen das Immunsystem als wichtiger Mediator in der Pathogenese der CMT 1 Subtypen A, B und X identifiziert werden. Insbesondere Makrophagen spielen eine tragende Rolle bei dem Verlust der axonalen Integrität, bei der Schädigung der Myelinscheiden, sowie bei der Dedifferenzierung von Schwann-Zellen. Entscheidender Faktor für Proliferation und Aktivierung der Makrophagen ist hierbei das Zytokin CSF-1, dessen korrespondierender Rezeptor auf Makrophagen exprimiert wird. Der CSF-1/CSF1R Signalweg bietet somit einen vielversprechenden Angriffspunkt.
In der vorliegenden Arbeit wurden Mausmodelle der CMT 1 Subtypen A, B und X mit einem niedermolekularen CSF-1-Rezeptor Inhibitor behandelt. Anschließend erfolgte eine funktionelle und strukturelle Auswertung der peripheren Nerven.
Das beste Ansprechen auf die Therapie zeigten Cx32def Mutanten. Strukturell fielen ein verringerter axonaler Schaden und eine verbesserte axonale Regenerationsfähigkeit sowie erhaltene neuromuskuläre Synapsen auf. Funktionell äußerte sich dies in verbesserten elektrophysiologischen Parametern und einem Krafterhalt, welcher als klinischer Parameter die größte Relevanz für betroffene Patienten hat und somit besonders hervorzuheben ist.
Auch P0het Mutanten zeigten Verbesserungen nach der CSF1RI Behandlung. Anders als bei Cx32def Tieren zeigte sich hier jedoch vor allem ein Erhalt der Myelinintegrität. Weiterhin wirkte sich die Therapie positiv auf elektrophysiologische Parameter und Krafttests aus. Vor allem besonders stark betroffene Individuen schienen hierbei von der CSF1RI Behandlung zu profitieren.
Bei PMP22tg Mutanten hingegen konnten keine positiven Effekte der CSF1RI Behandlung nachgewiesen werden. Strukturelle und funktionelle Parameter behandelter Tiere unterschieden sich nicht von unbehandelten.
Diese Ergebnisse unterstreichen die Relevanz der sekundären Entzündungsreaktion in CMT 1 Neuropathien als wichtigen Mediator in der Pathogenese. Weiterhin konnte gezeigt werden, dass eine Intervention im CSF-1/CSF1R Signalweg einen vielversprechenden möglichen Ansatz für die Therapie der bisher nicht behandelbaren CMT 1 Subypen X und B darstellt. Unausweichlich ist hierbei ein möglichst früher Therapiestart vor Ausprägung der ersten molekularen und histologischen Veränderungen. Im Hinblick auf die nicht die Lebenserwartung reduzierende Erkrankung muss ferner eine Minimierung der Nebenwirkungen der Therapie gewährleistet sein. Besonders hervorzuheben ist hier die Verwendung eines Inhibitors, welcher nicht in das zentrale Nervensystem vordringen kann und somit die Funktion der Mikroglia nicht beeinträchtigt.
Staphylococcus aureus asymptomatically colonizes the skin and anterior nares of 20-30% of the healthy human population. As an opportunistic human pathogen it elicits a variety of infections ranging from skin and soft tissue infections to highly severe manifestations such as pneumonia, endocarditis and osteomyelitis. Due to the emergence of multi resistant strains, treatment of staphylococcal infections becomes more and more challenging and the WHO therefore classified S. aureus as a “superbug”. The variety of diseases triggered by S. aureus is the result of a versatile expression of a large set of virulence factors. The most prominent virulence factor is the cytotoxic and haemolytic pore-forming α-toxin whose expression is mediated by a complex regulatory network involving two-component systems such as the agr quorum-sensing system, accessory transcriptional regulators and alternative sigma-factors. However, the intricate regulatory network is not yet understood in its entirety. Recently, a transposon mutation screen identified the AraC-family transcriptional regulator ‘Repressor of surface proteins’ (Rsp) to regulate haemolysis, cytotoxicity and the expression of various virulence associated factors. Deletion of rsp was accompanied by a complete loss of transcription of a 1232 nt long non-coding RNA, SSR42.
This doctoral thesis focuses on the molecular and functional characterization of SSR42. By analysing the transcriptome and proteome of mutants in either SSR42 or both SSR42 and rsp, as well as by complementation of SSR42 in trans, the ncRNA was identified as the main effector of Rsp-mediated virulence. Mutants in SSR42 exhibited strong effects on transcriptional and translational level when compared to wild-type bacteria. These changes resulted in phenotypic alterations such as strongly reduced haemolytic activity and cytotoxicity towards epithelial cells as well as reduced virulence in a murine infection model. Deletion of SSR42 further promoted the formation of small colony variants (SCV) during long term infection of endothelial cells and demonstrated the importance of this molecule for intracellular bacteria. The impact of this ncRNA on staphylococcal haemolysis was revealed to be executed by modulation of sae mRNA stability and by applying mutational studies functional domains within SSR42 were identified.
Moreover, various stressors modulated the transcription of SSR42 and antibiotic challenge resulted in SSR42-dependently increased haemolysis and cytotoxicity. Transcription of SSR42 itself was found under control of various important global regulators including AgrA, SaeS, CodY and σB, thereby illustrating a central position in S. aureus virulence gene regulation.
The present study thus demonstrates SSR42 as a global virulence regulatory RNA which is important for haemolysis, disease progression and adaption of S. aureus to intracellular conditions via formation of SCVs.
Die Konzertlandschaft im Deutschen Kaiserreich wurde neben den zahlreichen Opern- und Sinfonieorchestern vor allem durch die vielseitigen Darbietungen der Militärkapellen bei verschiedensten Anlässen geprägt. Dabei zeigten sich die Musikkorps als vielseitig einsetzbare Formationen und konnten einen spezifischen Beitrag zur Musikkultur leisten. Die Militärorchester hatten durch ihr konzertantes Wirken die Möglichkeit, großen Einfluss auf das musikkulturelle Profil innerhalb einer Region zu nehmen. Diese und andere Aspekte können am Beispiel der „Turnhallenkonzerte“ zu Arolsen aufgezeigt werden. Militärkapellmeister Hugo Rothe konnte sich bei diesen Konzerten sowohl durch seine Programmauswahl als auch durch die Zusammenarbeit mit vielen nationalen und internationalen Gastsolisten auszeichnen. Diese Ergänzung zum militärmusikalischen Tagesgeschäft (Paraden, Zeremonielle) war eine notwendige Facette, die sich besonders aus einem eigenen künstlerischen Ehrgeiz heraus zur musikalischen Selbstverwirklichung – sowohl für den Dirigenten als auch für die Orchestermusiker – entwickelte. Auf diese Weise konnte musikalische Volksbildung durch Popularisierung verschiedenster Werke sowie durch ein speziell zusammengestelltes Repertoire erfolgen, was sich anhand des umfangreichen Nachlassmaterials von Militärkapellmeister Hugo Rothe veranschaulichen lässt.
Das Ziel der vorliegenden Studie war es, die von Earle et. al im Jahr 2005 aufgestellten Qualitätsindikatoren für die Versorgung am Lebensende bei Patienten mit fortgeschrittenen und metastasierten Kopf- Hals- Tumoren nach Inbetriebnahme der Palliativstation am UKW im November 2009 zu überprüfen. Es wurden klinische retrospektive Daten derjenigen Patienten analysiert, bei denen in der ersten Jahreshälfte von 2008- 2011 ein fortgeschrittener oder rezidivierender bzw. metastasierender KHT diagnostiziert worden war. Die Verläufe von 208 Patienten wurden ausgewertet.Im Vergleich der beiden Gruppen zeigte sich nach Eröffnung der Palliativstation im November 2009 keine Veränderungen in der Rate der Patienten mit Tumortherapie in den letzten 2 Lebenswochen (14% präPF vs. 16% postPF, p=0,707; 8,7% präRM vs. 21,4% postRM, p=0,201). Ein ähnliches Bild zeigte die Analyse der Therapieumstellungen im letzten Lebensmonat (10% präPF vs. 8% postPF, p=0,828; 4,3% präRM vs. 9% postRM, p=0,485). Der Anteil der Patienten, die im letzten Lebensmonat in einer Notaufnahme vorstellig wurden, änderte sich nicht signifikant (22% präPF vs. 29% postPF, p=0,087; 34,8% präRM vs. 34% postRM, p=0,986) wie auch der Anteil Patienten, die im letzten Lebensmonat auf einer Intensivstation behandelt wurden (12% präPF vs. 16,5% postPF, p=0,479; 8,7% präRM vs. 16% postRM, p=0,485). Im Kohortenvergleich hatte der Anteil der Erkrankten mit einem fortgeschrittenen KHT mit Palliativkontakt nach Inbetriebnahme der Palliativstation nicht zugenommen (30% präPF vs. 34% postPF, p=0,622). Der Anteil der Patienten mit Rezidiven oder Metastasen mit Palliativkontakt hatte signifikant zugenommen (39% präRM vs. 59% postRM, p=0.05).
Diese retrospektive Studie ist als erster Schritt zu werten, die Auswirkungen des Ein-bezugs der spezialisierten Palliativversorgung bei fortgeschrittenen Kopf- Hals- Tumoren darzustellen. Insgesamt ist die Integration der Palliativmedizin insbesondere bei den primär metastasierten Patienten noch verbesserungswürdig.
Chronic Kidney Disease as an Important Co-morbid Condition in Coronary Heart Disease Patients

In patients with coronary heart disease (CHD) the control of the modifiable “traditional” cardiovascular risk factors such as hypertension, dyslipidemia, diabetes, achieving/maintaining normal body weight and smoking cessation is of major importance to improve prognosis. Guideline recommendations for secondary CHD prevention include specific treatment targets for blood pressure, lipid levels, and markers of glucose metabolism for both younger and older patients. Chronic kidney disease (CKD) has been identified as a “non-traditional” risk factor for worse outcome in CHD patients, as it is associated with a markedly increased risk for subsequent CV events and mortality.
The specific objectives of the current thesis-project are to investigate (a) the quality of care in a recent sample of German CHD patients and to investigate variation of risk factor control between younger and elder patients (≤70 versus >70 years), (b) to analyze the prevalence of CKD across Europe in stable CHD patients in the outpatient setting and during a hospital stay for CHD, (c) to investigate the level of awareness of CKD in German CHD patients and their treating physicians.
Data from the European-wide EUROASPIRE IV study were used that include data on 7998 CHD patients in the ambulatory setting (study visit) and during a hospital stay for CHD (index). The German EUROASPIRE IV study center in Würzburg recruited 536 patients in 2012-2013. Risk factor control was compared against the current recommendations of the European Society of Cardiology. CKD was described by stages of glomerular filtration rate (eGFR) and albuminuria. German patients were asked in an additional kidney specific module whether they have ever been told by a physician about renal impairment. The fact that CKD or acute kidney injury (AKI) was mentioned in prominent parts of the hospital discharge letter as well as correct ICD-coding of CKD or AKI served as a proxy for physician’s awareness of CKD.
The majority of German CHD patients was treated with the recommended drug therapies including e.g. β-blockers, anti-platelets and statins. However, treatment targets for blood pressure and LDL-cholesterol levels were not achieved in many patients (45% and 53%, respectively) and glycemic control in diabetic CHD patients with HbA1-levels <7% was insufficient (61%). A minority of patients reported on current smoking (10%), but unhealthy life-styles e.g. overweight/obesity (85%/37%) were frequent. Patterns of care differed between younger and older CHD patients while older patients were less likely to receive the recommended medical CHD-therapy, were more likely to have uncontrolled blood pressure and also to be diabetic. However, a greater proportion of diabetic patients >70 years was achieving the HbA1c target, and less elder patients were current smokers or were obese. About 17% of patients on average had CKD (eGFR< 60 ml/min/1.73m²) in the entire European sample at the study visit, and an additional 10% had albuminuria despite preserved eGFR, with considerable variation among countries. Impaired kidney function was observed in every fifth patient admitted for CHD in the entire European dataset of the EUROASPIRE IV study. Of the German CHD patients with CKD at the study visit, only a third were aware of their renal impairment. A minority of these patients was being seen by nephrologists, however, with a higher likelihood of CKD awareness and specialist care in more advanced stages of CKD. About a third of patients admitted for CHD showed either CKD or AKI during the hospital stay, but the discharge letter mentioned chronic or acute kidney disease only in every fifth of these patients. In contrast, correct ICD coding of CKD or AKI was more complete, but still suboptimal.
In summary, quality of secondary prevention in German CHD patients indicates considerably room for improvement, with life-style modifications may become an even greater factor in prevention campaigns than medical treatment into certain target ranges. Preventive therapies should also consider different needs in older individuals acknowledging physical and mental potential, other comorbidities and drug-interactions with co-medication. CKD is common in CHD patients, not only in the elderly. Since CHD and CKD affect each other and impact on worse prognosis of each other, raising the awareness of CKD among patients and physicians and considering CKD in medical therapy may improve prognosis and slow disease progression of CHD as well as CKD.
The current treatment strategies for diseases are assessed on the basis of diagnosed phenotypic changes due to an accumulation of asymptomatic events in physiological processes. Since a diagnosis can only be established at advanced stages of the disease, mainly due to insufficient early detection possibilities of physiological disorders, doctors are forced to treat diseases rather than prevent them. Therefore, it is desirable to link future therapeutic interventions to the early detection of physiological changes. So-called sensor-effector systems are designed to recognise disease-specific biomarkers and coordinate the production and delivery of therapeutic factors in an autonomous and automated manner. Such approaches and their development are being researched and promoted by the discipline of synthetic biology, among others.
Against this background, this paper focuses on the in vitro design of cytokine-neutralizing sensor-effector cells designed for the potential treatment of recurrent autoimmune diseases, especially rheumatoid arthritis.
The precise control of inducible gene expression was successfully generated in human cells. At first, a NF-κB-dependent promoter was developed, based on HIV-1 derived DNA-binding motives. The activation of this triggerable promoter was investigated using several inducers including the physiologically important NF-κB inducers tumor necrosis factor alpha (TNFα) and interleukin 1 beta (IL-1β). The activation strength of the NF-κB-triggered promoter was doubled by integrating a non-coding RNA. The latter combined expressed RNA structures, which mimic DNA by double stranded RNAs and have been demonstrated to bind to p50 or p65 by previous publications. The sensitivity was investigated for TNFα and IL-1β. The detection limit and the EC50 values were in in the lower picomolar range. Besides the sensitivity, the reversibility and dynamic of the inducible system were characterized. Hereby a close correlation between pulse times and expression profile was shown.
The optimized NF-κB-dependent promoter was then coupled to established TNFα- and IL-1-blocking biologicals to develop sensor-effector systems with anti-inflammatory activity, and thus potential use against autoimmune diseases such as rheumatoid arthritis. The biologicals were differentiated between ligand-blocking and receptor-blocking biologicals and different variants were selected: Adalimumab, etanercept and anakinra. The non-coding RNA improved again the activation strength of NF-κB-dependent expressed biologicals, indicating its universal benefit. Furthermore, it was shown that the TNFα-induced expression of NF-κB-regulated TNFα-blocking biologics led to an extracellular negative feedback loop. Interestingly, the integration of the non-coding RNA and this negative feedback loop has increased the dynamics and reversibility of the NF-κB-regulated gene expression. The controllability of drug release can also be extended by the use of inhibitors of classical NF-κB signalling such as TPCA-1. The efficacy of the expressed biologicals was detected through neutralization of the cytokines using different experiments. For future in vivo trials, first alginate encapsulations of the cells were performed. Furthermore, the activation of NF-κB-dependent promoter was demonstrated using co-cultures with human plasma samples or using synovial liquids.
With this generated sensor-effector system we have developed self-adjusting cytokine neutralizer cells as a closed-loop delivery system for anit-inflammatory biologics.
Insulin-like growth factor-I (IGF-I) is a 70-amino acid polypeptide with a molecular weight of approximately 7.6 kDa acting as an anabolic effector. It is essential for tissue growth and remodeling. Clinically, it is used for the treatment of growth disorders and has been proposed for various other applications including musculoskeletal diseases. Unlike insulin, IGF-I is complexed to at least six high-affinity binding proteins (IGFBPs) exerting homeostatic effects by modulating IGF-I availability to its receptor (IGF-IR) on most cells in the body as well as changing the distribution of the growth factor within the organism.1-3 Short half-lived IGF-I have been the driving forces for the design of localized IGF-I depot systems or protein modification with enhanced pharmacokinetic properties. In this thesis, we endeavor to present a versatile biologic into which galenical properties were engineered through chemical synthesis, e.g., by site-specific coupling of biomaterials or complex composites to IGF-I. For that, we redesigned the therapeutic via genetic codon expansion resulting in an alkyne introduced IGF-I, thereby becoming a substrate for biorthogonal click chemistries yielding a site-specific decoration.
In this approach, an orthogonal pyrrolysine tRNA synthetase (PylRS)/tRNAPyl CUA pair was employed to direct the co-translational incorporation of an unnatural amino acid—¬propargyl-L-lysine (plk)—bearing a clickable alkyne functional handle into IGF-I in response to the amber stop codon (UAG) introduced into the defined position in the gene of interest. We summarized the systematic optimization of upstream and downstream process alike with the ultimate goal to increase the yield of plk modified IGF-I therapeutic, from the construction of gene fusions resulting in (i) Trx-plk-IGF-I fusion variants, (ii) naturally occurring pro-IGF-I protein (IGF-I + Ea peptide) (plk-IGF-I Ea), over the subsequent bacterial cultivation and protein extraction to the final chromatographic purification. The opportunities and hurdles of all of the above strategies were discussed. Evidence was provided that the wild-type IGF-I yields were pure by exploiting the advantages of the pHisTrx expression vector system in concert with a thrombin enzyme with its highly specific proteolytic digestion site and multiple-chromatography steps. The alkyne functionality was successfully introduced into IGF-I by amber codon suppression. The proper folding of plk-IGF-I Ea was assessed by WST-1 proliferation assay and the detection of phosphorylated AKT in MG-63 cell lysate. The purity of plk-IGF-I Ea was monitored with RP-HPLC and SDS-PAGE analysis. This work also showed site-specific coupling an alkyne in plk-IGF-I Ea by copper (I)-catalyzed azide-alkyne cycloaddition (CuAAC) with potent activities in vitro. The site-specific immobilization of plk-IGF-I Ea to the model carrier (i.e., agarose beads) resulted in enhanced cell proliferation and adhesion surrounding the IGF-I-presenting particles. Cell proliferation and differentiation were enhanced in the accessibility of IGF-I decorated beads, reflecting the multivalence on cellular performance.
Next, we aimed at effectively showing the disease environment by co-delivery of fibroblast growth factor 2 (FGF2) and IGF-I, deploying localized matrix metalloproteinases (MMPs) upregulation as a surrogate marker driving the response of the drug delivery system. For this purpose, we genetically engineered FGF2 variant containing an (S)-2-amino-6-(((2-azidoethoxy)carbonyl)amino)hexanoic acid incorporated at its N-terminus, followed by an MMPs-cleavable linker (PCL) and FGF2 sequence, thereby allowing site-directed, specific decoration of the resultant azide-PCL-FGF2 with the previously mentioned plk-IGF-I Ea to generate defined protein-protein conjugates with a PCL in between. The click reaction between plk-IGF-I Ea and azide-PCL-FGF2 was systematically optimized to increase the yield of IGF-FGF conjugates, including reaction temperature, incubation duration, the addition of anionic detergent, and different ratios of the participating biopharmaceutics. The challenge here was that CuAAC reaction components or conditions might oxidize free cysteines of azide-PCL-FGF2 and future work needs to present the extent of activity retention after conjugation. Furthermore, our study provides potential options for dual-labeling of IGF-I either by the introduction of unnatural amino acids within two distinct positions of the protein of interest for parallel “double-click” labeling of the resultant plk-IGF-I Ea-plk or by using a combination of enzymatic-catalyzed and CuAAC bioorthogonal coupling strategies for sequentially dual-labeling of plk-IGF-I Ea.
In conclusion, genetic code expansion in combination with click-chemistry provides the fundament for novel IGF-I analogs allowing unprecedented site specificity for decoration. Considerable progress towards IGF-I based therapies with enhanced pharmacological properties was made by demonstrating the feasibility of the expression of plk incorporated IGF-I using E. coli and retained activity of unconjugated and conjugated IGF-I variant. Dual-labeling of IGF-I provides further insights into the functional requirements of IGF-I. Still, further investigation warrants to develop precise IGF-I therapy through unmatched temporal and spatial regulation of the pleiotropic IGF-I.
Ovarian cancer is one of the most common gynecological malignancies in the world. The prevalence of a microbial signature in ovarian cancer has been reported by several studies till date. In these microorganisms, Human herpesvirus 6 (HHV-6) and Chlamydia trachomatis (C.tr) are especially important as they have significantly high prevalence rate. Moreover, these pathogens are directly involved in causing DNA damage and thereby disrupting the integrity of host genome which is the underlying cause of any cancer. This study focuses on how the two pathogens, HHV-6 and C. trachomatis can affect the genome integrity in their individual capacities and thereby may drive ovarian epithelial cells towards transformation. HHV-6 has unique tendency to integrate its genome into the host genome at subtelomeric regions and achieve a state of latency. This latent virus may get reactivated during the course of life by stress, drugs such as steroids, during transplantation, pregnancy etc. The study presented here began with an interesting observation wherein the direct repeat (DR) sequences flanking the ends of double stranded viral genome were found in unusually high numbers in human blood samples as opposed to normal ratio of two DR copies per viral genome. This study was corroborated with in vitro data where cell lines were generated to mimic the HHV-6 status in human samples. The same observation of unusually high DR copies was found in these cell lines as well. Interestingly, fluorescence in situ hybridization (FISH) and inverse polymerase chain reaction followed by southern blotting showed that DR sequences were found to be integrated in nontelomeric regions as opposed to the usual sub-telomeric integration sites in both human samples and in cell lines. Sanger sequencing confirmed the non-telomeric integration of viral DR sequences in the host genome. Several studies have shown that C. trachomatis causes DNA damage and inhibits the signaling cascade of DNA damage response. However, the effect of C. trachomatis infection on process of DNA repair itself was not addressed. In this study, the effect of C. trachomatis infection on host base excision repair (BER) has been addressed. Base excision repair is a pathway which is responsible for replacing the oxidized bases with new undamaged ones. Interestingly, it was found that C. trachomatis infection downregulated polymerase β expression and attenuated polymerase β- mediated BER in vitro. The mechanism of the polymerase β downregulation was found to be associated with the changes in the host microRNAs and downregulation of tumor suppressor, p53. MicroRNA-499 which has a binding site in the polymerase β 3’UTR was shown to be upregulated during C. trachomatis infection. Inhibition of miR-499 using synthetic miR-499 inhibitor indeed improved the repair efficiency during C. trachomatis infection in the in vitro repair assay. Moreover, p53 transcriptionally regulates polymerase β and stabilizing p53 during C. trachomatis infection enhanced the repair efficiency. Previous studies have shown that C. trachomatis can reactivate latent HHV-6. Therefore, genomic instability due to insertions of unstable ‘transposon-like’ HHV-6 DR followed by compromised BER during C. trachomatis infection cumulatively support the hypothesis of pathogenic infections as a probable cause of ovarian cancer
Statistical Procedures for modelling a random phenomenon heavily depend on the choice of a certain family of probability distributions. Frequently, this choice is governed by a good mathematical feasibility, but disregards that some distribution properties may contradict reality. At most, the choosen distribution may be considered as an approximation. The present thesis starts with a construction of distributions, which uses solely available information and yields distributions having greatest uncertainty in the sense of the maximum entropy principle. One of such distributions is the monotonic distribution, which is solely determined by its support and the mean. Although classical frequentist statistics provides estimation procedures which may incorporate prior information, such procedures are rarely considered. A general frequentist scheme for the construction of shortest confidence intervals for distribution parameters under prior information is presented. In particular, the scheme is used for establishing confidence intervals for the mean of the monotonic distribution and compared to classical procedures. Additionally, an approximative procedure for the upper bound of the support of the monotonic distribution is proposed. A core purpose of auditing sampling is the determination of confidence intervals for the mean of zero-inflated populations. The monotonic distribution is used for modelling such a population and is utilised for the procedure of a confidence interval under prior information for the mean. The results are compared to two-sided intervals of Stringer-type.
Unterschiede in Frontaler Kortex Oxygenierung in zweierlei Risikogruppen der Alzheimer Demenz

Die verbesserte medizinische Versorgung führt zu einer zunehmenden Lebenserwartung unserer Gesellschaft. Damit steigt auch die sozioökonomische Relevanz neurodegenerativer Erkrankungen kontinuierlich. Für die Alzheimer Demenz (AD), die dabei die häufigste Ursache darstellt, stehen bisher keine krankheitsmodifizierenden Behandlungsoptionen zur Verfügung. Die lange präklinische Phase der Erkrankung birgt jedoch großes Potential für die Entwicklung neuer Behandlungsoptionen. Das Untersuchen von Risikogruppen ist für die Identifikation von Prädiktoren einer späteren AD Manifestation von besonderem Interesse. In diesem Zusammenhang werden insbesondere das Vorliegen genetischer Risikokonstellationen, wie dem Apolipoprotein E (APOE) Ɛ4-Allel, sowie kognitiver Risikofaktoren, wie der „leichten kognitiven Beeinträchtigung“ (MCI), diskutiert. Die Identifikation präklinischer Aktivierungsunterschiede in relevanten Gehirnregionen von Risikogruppen kann als Basis für die Entwicklung neurofunktioneller Früherkennungs-Marker dienen. Der präfrontale Kortex (PFC), welcher mit der Steuerung von Exekutivfunktionen assoziiert wird, hat sich in diesem Zusammenhang in bisherigen Studien als eine relevante Schlüsselregion manifestiert. Aufgrund der aufwendigen und kostenintensiven bildgebenden Untersuchungsmethoden, sind die genauen Prozesse jedoch noch unklar.
Ziel der vorliegenden Arbeit war es daher, Unterschiede in der PFC Oxygenierung in zweierlei Risikogruppen der AD mit einer kostengünstigeren Bildgebungsmethode, der funktionellen Nahinfrarot Spektroskopie (fNIRS), zu untersuchen. Dafür wurde in einem ersten Schritt, der Trailmaking Test (TMT), ein weitverbreiteter neuropsychologischer Test zur Erfassung exekutiver Funktionen, für fNIRS implementiert. Als Grundlage für die Untersuchung frühpathologischer Prozesse, wurden zunächst gesunde Alterungsprozesse betrachtet. Der Vergleich von jungen und älteren Probanden (n = 20 pro Gruppe) wies neben der Eignung der Testimplementierung für fNIRS auf eine spezifische bilaterale PFC Oxygenierung hin, welche bei jungen Probanden rechtshemisphärisch lateralisiert war. Ältere Probanden hingegen zeigten bei vergleichbaren Verhaltensdaten insgesamt mehr signifikante Kanäle sowie eine Abnahme der Lateralisierung. Dies kann als zusätzlicher Bedarf an Ressourcen in gesunden Alterungsprozessen interpretiert werden.
Im Rahmen der Hauptstudie wurden anschließend insgesamt 604 ältere Probanden im Alter von 70 bis 76 Jahren untersucht. Zunächst wurde die genetische Risikogruppe der Ɛ4-Allel-Träger (n = 78) mit den neutralen Ɛ3-Allel-Trägern (n = 216) und den Trägern des als protektiv geltenden Ɛ2-Allels (n = 50) verglichen. Hierbei zeigte sich eine geringere Oxygenierung der Risikogruppe bei geringer Aufgabenschwierigkeit, während sich ein erhöhter Oxygenierungsanstieg im medialen PFC mit steigender Aufgabenschwierigkeit zeigte. Dies deutet auf einen erhöhten Bedarf an neuronalen Kontrollmechanismen der Risikogruppe zur Bewältigung der steigenden Aufgabenschwierigkeit hin. Die protektive Gruppe zeigte hingegen eine erhöhte Oxygenierung im ventralen PFC mit steigender Aufgabenschwierigkeit, was möglicherweise auf einen präventiven Effekt hindeuten könnte.
Weiterführend wurden MCI-Patienten mit gesunden Probanden (n = 57 pro Gruppe) hinsichtlich des kognitiven Risikofaktors verglichen. Hierbei zeigte sich ein punktuell reduzierter Oxygenierunganstieg der MCI Patienten mit steigender Aufgabenschwierigkeit vor allem im ventralen PFC bei ebenfalls stabiler Verhaltensleistung. Die gefundene Reduktion könnte ein Zeichen für eine aufgebrauchte kognitive Reserve sein, welche Einbußen auf Verhaltensebene voranzugehen scheint.
Diese charakteristischen Unterschiede in den frontalen Oxygenierungsmustern von Risikogruppen (APOE, MCI) könnten als Biomarker zur Früherkennung von AD noch vor dem Auftreten kognitiver Einbußen dienen. Die fNIRS-Untersuchung während der Durchführung des TMT hat sich in diesem Zusammenhang als potentielles Instrument zur Frühdiagnose der präklinischen Phase der AD als geeignet erwiesen. Die Ergebnisse werden unter Einbezug des wissenschaftlichen Kontexts interpretiert und Implikationen für weitere notwendige Studien sowie die klinische Anwendbarkeit diskutiert.
All living organisms need timekeeping mechanisms to track and anticipate cyclic changes in their environment. The ability to prepare for and respond to daily and seasonal changes is endowed by circadian clocks. The systemic features and molecular mechanisms that drive circadian rhythmicity are highly conserved across kingdoms. Therefore, Drosophila melanogaster with its relatively small brain (ca. 135.000 neurons) and the outstanding genetic tools that are available, is a perfect model to investigate the properties and relevance of the circadian system in a complex, but yet comprehensible organism.
The last 50 years of chronobiological research in the fruit fly resulted in a deep understanding of the molecular machinery that drives circadian rhythmicity, and various histological studies revealed the neural substrate of the circadian system. However, a detailed neuroanatomical and physiological description on the single-cell level has still to be acquired. Thus, I employed a multicolor labeling approach to characterize the clock network of Drosophila melanogaster with single-cell resolution and additionally investigated the putative in- and output sites of selected neurons.
To further study the functional hierarchy within the clock network and to monitor the “ticking clock“ over the course of several circadian cycles, I established a method, which allows us to follow the accumulation and degradation of the core clock genes in living brain explants by the means of bioluminescence imaging of single-cells.
Errors in Prospective Memory

Prospective memory is the ability to implement intentions at a later point in time in response to a specified cue. Such prospective memory tasks often occur in daily living and workplace situations. However, in contrast to retrospective memory there has been relatively little research on prospective memory. The studies by Harris (1984) and Einstein and MacDaniel (1990) served as a starting point for a now steadily growing area of research. Based on this emerging field of study this dissertation presents and connects and five journal articles, which further explore prospective memory by focusing on its potential errors.
The first article addresses the question if additional cognitive resources are needed after a prospective memory cue occurs to keep the intention active until it is implemented. The theory by Einstein, McDaniel, Williford, Pagan and Dismukes (2003), which suggested this active maintenance, could not be replicated. The second article demonstrated that interruptions between cue and the window of opportunity to implement the intention reduce prospective memory performance, especially if the interruption is tied with a change of context. Article three to five were focused on the erroneous implementation of a no longer active prospective memory task, so called commission errors. The suggested mechanism for their occurrence, the dual-mechanism account (Bugg, Scullin, & Rauvola, 2016), was not suited to explain the present results. A modification for the dual-mechanism account was formulated, which can account for prior work, as well as for the present data.
The results of all five articles also indicate that the moment of cue retrieval is even more relevant for prospective memory and its errors than previously accounted for.
Das humane Schädeldach besteht aus fünf Schädelplatten, die durch intramembranöse Ossifikation entstehen. Wenn diese in der Embryonalentwicklung aufeinandertreffen, bilden sich Schädelnähte aus, die eine Fusion der Schädelplatten verhindern und damit ein Schädelwachstum parallel zu Gehirnentwicklung ermöglichen. Für diesen Prozess ist eine Balance aus Zellproliferation und Differenzierung nötig, deren Aufrechterhaltung wiederum durch eine komplexe Regulation von verschiedenen Signalwegen gewährleistet wird. Störungen in diesem regulatorischen System können zu einer vorzeitigen Fusion der Schädelplatten, Kraniosynostose genannt, führen. Die Kraniosynostose ist eine der häufigsten kraniofazialen Fehlbildungen beim Menschen. Durch kompensatorisches Wachstum an den nicht fusionierten Suturen entstehen charakteristische Schädeldeformationen, die sekundär einen erhöhten intrakranialen Druck zur Folge haben können. Eine vorzeitige Fusion der Suturen kann sowohl isoliert als auch syndromal zusammen mit weiteren klinischen Auffälligkeiten vorliegen. Bisher sind über 150 verschiedene Kraniosynostose Syndrome beschrieben und insgesamt 25-30% aller Kraniosynostose Patienten sind von einer syndromalen Form betroffen. Da die klinischen Merkmale der Kraniosynostose Syndrome variabel sind und zum Teil überlappen, ist eine klare klinische Diagnose häufig erschwert. Sowohl Umwelteinflüsse als auch genetische Veränderungen können die Ursache für Kraniosynostosen sein. Vor allem bei syndromalen Kraniosynostosen wurden genetische Veränderungen, wie beispielsweise Mutationen in den Genen FGFR2, FGFR3, TWIST1 und EFNB1, identifiziert. Darüber hinaus wurden chromosomale Veränderungen wie partielle Monosomien von 7p, 9p oder 11p sowie partielle Trisomien von 5q, 13q oder 15q mit Kraniosynostose assoziiert. Trotzdem ist in über 50% der Fälle die genetische Ursache unbekannt und die Pathogenese von Kraniosynostosen noch nicht vollständig geklärt.
Ziel dieser Arbeit war es neue genetische Ursachen bei Kraniosynostose Patienten zu identifizieren und so zur Aufklärung der Pathogenese beizutragen. Es wurde die genomische DNA von 83 Patienten molekulargenetisch durch Mikroarray basierte vergleichende Genomhybridisierung (Array-CGH) oder durch ein speziell entworfenes Next Generation Sequencing (NGS) Genpanel untersucht. Bei 30% der Patienten konnte eine potentiell pathogene Veränderung identifiziert werden. Davon waren 23% chromosomale Aberrationen wie unbalancierte Translokationen, isolierte interstitielle Verluste und ein Zugewinn an genomischen Material. Bei zwei Patienten wurden unbalancierte Translokationen mit partieller 5q Trisomie nachgewiesen. Das Gen MSX2 liegt innerhalb des duplizierten Bereichs, sodass möglicherweise eine MSX2 Überexpression vorliegt. Für ein normales Schädelwachstum ist jedoch die richtige Menge an MSX2 kritisch. Des Weiteren wurde eine partielle Deletion von TCF12 detektiert, die in einer Haploinsuffizienz von TCF12 resultiert. TCF12 Mutationen sind mit Koronarnahtsynosten assoziiert. In einem anderen Fall lag das Gen FGF10 innerhalb der duplizierten 5p15.1-p12 Region. Das Gen kodiert für einen Liganden des FGF Signalwegs und wurde bisher noch nicht mit Kraniosynostose assoziiert. Aufgrund dessen wurden Analysen im Tiermodell Danio rerio durchgeführt. Eine simulierte Überexpression durch Injektion der fgf10a mRNA in das 1-Zell Stadium führte zu schweren Gehirn-, Herz- und Augendefekten.
Mittels NGS wurden 77% der potentiell pathogenen genetischen Veränderungen identifiziert. Hierfür wurde in dieser Arbeit ein Genpanel erstellt, das 68 Gene umfasst. Es wurden sowohl bekannte Kraniosynostose- als auch Kandidaten-Gene sowie Gene, die mit der Ossifikation assoziiert sind, in die Analyse eingeschlossen. Das Genpanel wurde durch die Sequenzierung von fünf Kontrollproben mit bekannten Mutationen erfolgreich validiert. Anschließend wurde die genomische DNA von 66 Patienten analysiert. Es konnten 20 (potentiell) pathogene Varianten identifiziert werden. Neben bereits bekannten Mutationen in den Genen FGFR1, FGFR2, FGFR3 und TWIST1, konnten zusätzlich 8 neue, potentiell pathogene Varianten in den Genen ERF, MEGF8, MSX2, PTCH1 und TCF12 identifiziert werden. Die Ergebnisse dieser Arbeit tragen dazu bei das Mutationsspektrum dieser Gene zu erweitern. Bei zwei der Varianten handelte es sich um potentielle Spleißvarianten. Für diese konnte in einem in vitro Spleißsystem gezeigt werden, dass sie eine Änderung des Spleißmusters bewirken. Der Nachweis von zwei seltenen Varianten in den Genen FGFR2 und HUWE1 hat außerdem dazu beigetragen die Pathogenität dieser spezifischen Varianten zu bekräftigen. Eine Variante in POR, die aufgrund bioinformatischer Analysen als potentiell pathogen bewertet wurde, wurde nach der Segregationsanalyse als wahrscheinlich benigne eingestuft. Zusammenfassend konnten bei etwa einem Drittel der Patienten, die mit dem NGS Genpanel analysiert wurden, eine genetische Ursache identifiziert werden. Dieses Genpanel stellt somit ein effizientes diagnostisches Tool dar, das zukünftig in der genetischen Routine-Diagnostik von Kraniosynostose-Patienten eingesetzt werden kann. Die Ergebnisse dieser Arbeit zeigen, dass sowohl eine Untersuchung auf CNVs als auch auf Sequenzänderungen bei Kraniosynostose Patienten sinnvoll ist.

Biofabrication is an advancing new research field that might, one day, lead to complex products like tissue replacements or tissue analogues for drug testing. Although great progress was made during the last years, there are still major hurdles like new types of materials and advanced processing techniques. The main focus of this thesis was to help overcoming this hurdles by challenging and improving existing fabrication processes like extrusion-based bioprinting but also by developing new techniques. Furthermore, this thesis assisted in designing and processing materials from novel building blocks like recombinant spider silk proteins or inks loaded with charged nanoparticles.
A novel 3D printing technique called Melt Electrospinning Writing (MEW) was used in Chapter 3 to create tubular constructs from thin polymer fibers (roughly 12 μm in diameter) by collecting the fibers onto rotating and translating cylinders. The main focus was put on the influence of the collector diameter and its rotation and translation on the morphology of the constructs generated by this approach. In a first step, the collector was not moving and the pattern generated by these settings was analyzed. It could be shown that the diameter of the stationary collectors had a big impact on the morphology of the constructs. The bigger the diameter of the mandrel (smallest collector diameters 0.5 mm, biggest 4.8 mm) got, the more the shape of the generated footprint converged into a circular one known from flat collectors. In a second set of experiments the mandrels were only rotated. Increasing the rotational velocity from 4.2 to 42.0 rpm transformed the morphology of the constructs from a figure-of-eight pattern to a sinusoidal and ultimately to a straight fiber morphology. It was possible to prove that the transformation of the pattern was comparable to what was known from increasing the speed using flat collectors and that at a critical speed, the so called critical translation speed, straight fibers would appear that were precisely stacking on top of each other. By combining rotation and translation of the mandrel, it was possible to print tubular constructs with defined winding angles. Using collections speeds close to the critical translation speed enabled higher control of fiber positioning and it was possible to generate precisely stacked constructs with winding angles between 5 and 60°.
In Chapter 4 a different approach was followed. It was based on extrusion-based bioprinting in combination with a hydrogel ink system. The ink was loaded with nanoparticles and the nanoparticle release was analyzed. In other words, two systems, a printable polyglycidol/hyaluronic acid ink and mesoporous silica nanoparticles (MSN), were combined to analyze charge driven release mechanism that could be fine-tuned using bioprinting. Thorough rheological evaluations proved that the charged nanoparticles, both negatively charged MSN-COOH and positively charged MSN-NH2, did not alter the shear thinning properties of the ink that revealed a negative base charge due to hyaluronic acid as one of its main components. Furthermore, it could be shown that the particles did also not have a negative effect on the recovery properties of the material after exposure to high shear. During printing, the observations made via rheological testing were supported by the fact that all materials could be printed at the same settings of the bioprinter. Using theses inks, it was possible to make constructs as big as 12x12x3 mm3 composed of 16 layers. The fiber diameters produced were about 627±31 μm and two-component constructs could be realized utilizing the two hydrogel print heads of the printer to fabricate one hybrid construct. The particle distribution within those constructs was homogeneous, both from a microscopic and a macroscopic point of view. Particle release from printed constructs was tracked over 6 weeks and revealed that the print geometry had an influence on the particle release. Printed in a geometry with direct contact between the strands containing different MSN, the positively charged particles quickly migrated into the strand previously containing only negatively charged MSN-COOH. The MSN-COOH seemed to be rather released into the surrounding liquid and also after 6 weeks no MSN-COOH signal could be detected in the strand previously only containing MSN-NH2. In case of a geometry without direct contact between the strands, the migration of the positively charged nanoparticles into the MSN-COOH containing strand was strongly delayed. This proved that the architecture of the printed construct can be used to fine-tune the particle release from nanoparticle containing printable hydrogel ink systems.
Chapter 5 discusses an approach using hydrogel inks based on recombinant spider silk proteins processed via extrusion-based bioprinting. The ink could be applied for printing at protein concentrations of 3 % w/v without the addition of thickeners or any post process crosslinking. Both, the recombinant protein eADF4(C16) and a modification introducing a RGD-sequence to the protein (eADF4(C16)-RGD), could be printed revealing a very good print fidelity. The RGD modification had positive effect on the adhesion of cells seeded onto printed constructs. Furthermore, human fibroblasts encapsulated in the ink at concentrations of 1.2 million cells per mL did not alter the print fidelity and did not interfere with the crosslinking mechanism of the ink. This enabled printing cell laden constructs with a cell survival rate of 70.1±7.6 %. Although the cell survival rate needs to be improved in further trials, the approach shown is one of the first leading towards the shift of the window of biofabrication because it is based on a new material that does not need potentially harmful post-process crosslinking and allows the direct encapsulation of cells staying viable throughout the print process.
Hintergrund dieser Doktorarbeit ist die ungeklärte Pathophysiologie der idiopathischen Dystonie. Die DYT1 Dystonie ist die häufigste hereditäre Dystonie und weist eine Mutation im Tor1a-Gen auf, welches das Protein TorsinA kodiert. Diese DYT1 Dystonie besitzt interessanterweise lediglich eine Penetranz von 30%. Ein gutes Nagermodell mit einem klinisch dystonen Phänotyp existiert bislang nicht. Diese Dissertation adressiert die Frage, ob sich eine Dystonie bei DYT1 relevanter genetischer Prädisposition durch peripheren Stress („second-hit“) manifestiert.
Bei Tor1a +/- Mäusen (50% TorsinA Expression, Tor1a +/-), die im naiven Zustand keinen dystonen Phänotyp haben, sowie bei Wildtyp (wt) Kontrolltieren im Alter von vier Monaten wurde eine rechtsseitige reversible N. ischiadicus Quetschläsion durchgeführt. Die Tiere wurden daraufhin in einem Beobachtungszeitraum von acht Wochen nach dem Trauma verhaltensanalytisch und morphologisch untersucht.
Folgende Ergebnisse wurden hierbei erzielt: Im „Tail-suspension-Test“ zeigte sich bereits ein Tag nach der Quetschläsion des N. ischiadicus eine passagere Parese des betroffenen rechten Hinterbeins bei wt und Tor1a +/- Mäusen. Die fokale Dystonie entwickelte sich ab der vierten Woche bei Tor1a +/- Mäusen stärker als bei den wt Kontrolltieren. Durch das computergestützte Ganganalysesystem (Catwalk™ XT 10.0) konnte bei wt und Tor1a +/- Tieren eine Woche nach der Quetschläsion eine Veränderung der Schrittfolgemuster mit einer Reduktion des Schrittfolge-Regularitäts-Index festgestellt werden. Die abnormale Schrittfolge beim Laufen führte bei Tor1a +/- Mäusen zu einer progredienten Abnahme des Schrittfolge-Regularitäts-Index, während sich wt Mäuse nach fünf Wochen aber wieder erholten. Bei der Überprüfung der Koordinationsfähigkeit beider Genotypen durch den Rotarod Test konnten keine signifikanten Unterschiede festgestellt werden. Immunhistochemische Färbungen des N. ischiadicus auf den Myelinmarker Myelinprotein Zero MPZ, den axonalen Marker Neurofilament und Makrophagen Marker F4/80+ vor der Nervenläsion und acht Wochen nach der Nervenläsion zeigten ebenso keine signifikanten Unterschiede zwischen wt und Tor1a +/- Tieren. Die Anzahl Nissl+ Neurone im lumbalen Rückenmark (L2-4), Striatum und zerebralen Kortex und zudem die Anzahl CD11b positiver Mikroglia im lumbalen Rückenmark (L2-4) wiesen ebenfalls keine signifikanten Unterschiede im Vergleich der beiden Genotypen nach Nervenläsion auf.
Abschließend wurden noch verschiedene Behandlungsexperimente durchgeführt, um zu klären, ob die gefundenen Unterschiede zwischen Tor1a +/- und Tor1a +/+ Mäusen dopaminerg verursacht sind. Hierfür wurden sowohl genetisch mutierte Tor1a +/- Mäuse als auch wt Mäuse nach der Quetschläsion entweder mit einem Kombinationspräparat aus L-Dopa und Benserazid oder mit AMPT acht Wochen lang behandelt.
Folgende Ergebnisse wurden hierbei ermittelt: Die Schrittfolgeregularität beim Catwalk™ XT 10.0 zeigte bei genetisch mutierten Mäusen eine deutliche Auswirkung der Medikation. Tor1a +/- AMPT Mäuse wiesen nach der Läsion eine progrediente Zunahme der Schrittfolgeregularität auf das Ausgangsniveau auf, Tor1a + /- L-Dopa Mäuse hingegen entwickelten auf die Läsion hin eine kontinuierliche Abnahme der Schrittfolgeregularität und konnten sich nicht auf ihr Ausgangsniveau erholen. Die Ergebnisse beim „Tail-suspension-Test“ zeigten ähnliche Resultate: Tor1a +/- Mäuse wiesen nach der Quetschläsion auf die Verabreichung von AMPT hin eine Reduktion der fokalen Dystonie auf, wohingegen die Behandlung mit L-Dopa bei Mutanten zu einer Verschlechterung der Dystonie führte.
Schlussfolgernd zeigt die vorliegende Dissertation, dass ein peripheres Trauma bei genetisch prädispositionierten Mäusen im Sinne eines „second-hits“ zur klinischen Ausprägung einer DYT1 Dystonie führt. Die verstärkte fokale Dystonie in Tor1a +/- Mäusen ist nicht durch ein unterschiedliches Ausmaß an Nervenschädigung nach N. ischiadicus Quetschläsion oder durch morphologische Veränderungen der Tor1a +/- Mäuse im Bereich des N. ischiadicus, Rückenmarks, Striatums oder des Kortex zu begründen.
Calcium phosphate cements (CPC) represent valuable synthetic bone grafts, as they are self-setting, biocompatible, osteoconductive and in their composition similar to the inorganic phase of human bone. Due to their long shelf-life, neutral setting and since water is sufficient for setting, hydroxyapatite (HA) forming cements are processed in different paste formulations. Those comprise dual setting, Ca2+ binding and premixed cement systems. With dual setting formulations, both dissolution and precipitation of the cement raw powder occur simultaneously to the polymerization of water-soluble monomers to form a hydrogel. Chelating agents are able to form complexes with Ca2+ released from the raw powder. Premixed systems mostly contain the raw powder of the cement and a non-aqueous binder liquid which delays the setting reaction until application in the moist physiological environment. In the present work, two of those reaction mechanisms allowed the development of HA based cement applications.
Drillable cements are of high clinical interest, as the quality of screw and plate osteosynthesis techniques can be improved by cement augmentation. A drillable, dual setting composite from HA and a poly(2-hydroxyethyl methacrylate) hydrogel was analyzed with respect to the influence of monomer content and powder-to-liquid ratio on setting kinetics and mechanical outcome. While the conversion to HA and crystal growth were constantly confined with increased monomer amount, a minimum concentration of 50 % was required to see impressive ameliorations including a low bending modulus and high fracture energy at improved bending strength. Increasing the liquid amount enabled injection of the paste as well as drilling after 10 min of pre-setting.
While classic bone wax formulations have drawbacks such as infection, inflammation, hindered osteogenesis and a lack of biodegradability, the as-presented premixed formulation is believed to exhibit outmatching properties. It consisted of HA raw powders and a non-aqueous, but water-miscible carrier liquid from poly(ethylene glycol) (PEG). The bone wax was proved to be cohesive and malleable, it withstood blood pressure conditions and among deposition in an aqueous environment, PEG was exchanged such that porous, nanocrystalline HA was formed. Incorporation of a model antibiotic proved the suitability of the novel bone wax formulation for drug release purposes.
Prefabricated laminates from premixed carbonated apatite forming cement and poly(ε-caprolactone) fiber mats with defined pore architecture were presented as a potential approach for the treatment of 2-dimensional, curved cranial defects. They are flexible until application and were produced in a layer-by-layer approach from both components such that the polymer scaffold prevents the cement from flowing. It was demonstrated that solution electrospinning with a patterned collector for the fabrication of perforated fiber mats was suitable, as high fiber volume contents in combination with an appropriate interface enabled the successful fabrication of mechanically reinforced laminates. Mild immersion of the scaffolds under alkaline conditions additionally improved the interphase followed by an increase in bending-strength.
Since few years, magnesium phosphate cements (MPC) have attracted increasing attention for bone replacement. Compared to CPC, MPC exhibit a higher degradation potential and high early strength and they release biologically valuable Mg2+. However, common systems offer some challenges while using them in non-classic cement formulations such as the need for foreign ion supply, the potential acidity of the reaction or the fast setting kinetics. Here, it was possible to develop a chelate-setting MPC paste with a broad spectrum of potential applications.
The general mechanism of the novel setting principle was tested in a proof-of-principle manner. The cement paste consisted of farringtonite with differently concentrated phytic acid solution for chelate formation with Mg2+ from the raw powder. Adjusting the phytic acid content and adding a magnesium oxide as setting regulator to compensate its retarding effect resulted in drillable formulations. Additionally, there is a strong clinical demand for well working bone adhesives especially in a moist environment. Mostly the existing formulations are non-biodegradable. Ex vivo adhesion of the above presented MPC under wet conditions on bone demonstrated over a course of 7 d shear strengths of 0.8 MPa. Further, the hardened cement specimens showed a mass loss of 2 wt.% within 24 d in an aqueous environment and released about 0.17 mg/g of osteogenic Mg2+ per day. Together with the demonstrated cytocompatibility towards human fetal osteoblasts, this cement system showed promising characteristics in terms of degradable biocements with special application purposes.
Diese Arbeit bietet erstmals eine Edition und Übersetzung handschriftlicher Aufzeichnungen aus dem chirurgischen Unterricht in Padua für angehende akademisch gebildete Ärzte im 16. Jahrhundert. Sie gibt damit detaillierte Einblicke in die chirurgische Lehre in Padua, der führenden medizinischen Fakultät des damaligen Europas. Johann Konrad Zinn, ein deutscher Medizinstudent, verfasste diese Mitschriften in der Chirurgie-Vorlesung, die Hieronymus Fabricius ab Aquapendente, einer der bekanntesten Anatomen jener Zeit, damals abhielt.
Die endovaskuläre Behandlung (EVAR) des abdominellen Aortenaneurysmas (AAA) hat sich den vergangenen Jahren zur Standardtherapie etabliert. Die Vielzahl an verfügbaren Stentgrafts wird in der Regel in Zulassungsstudien und Registern, die die Gefahr eines Selektionsbias beinhalten, evaluiert und spiegelt oftmals den klinischen Nutzen nur unzureichend wider. Ziel dieser Arbeit war es zu evaluieren, ob unter klinischen Alltagsbedingungen die kurz- und mittelfristigen Ergebnisse nach Endurant® Stentgraftimplantation mit denen des 1262 Patienten umfassenden ENGAGE-Registers vergleichbar sind.
Diese Arbeit beinhaltet die Daten der ersten, konsekutiven 100 Patienten (91,0 % Männer und mittleres Patientenalter von 73,1 ± 8,4 Jahren), die im Zeitraum Februar 2009 bis August 2014 mit dem Endurant® Stentgraft (Medtronic, Inc., Minneapolis, MN, USA) an der Universitätsklinik Würzburg unter klinischen Alltagsbedingungen behandelt wurden. Präoperativ wies das Patientenkollektiv einen mittleren Aortenaneurysmadurchmesser von 57,2 mm ± 11,9 mm, eine mittlere proximale Aortenhalslänge von 27,8 mm ± 13,7 mm und einen mittleren proximalen Aortenhalsdurchmesser von 24,0 mm ± 3,4 mm auf. Die infrarenale Angulation betrug 22,0 ± 15,6 Grad und war signifikant unterschiedlich zu ENGAGE. Die klinische Alltagssituation, die diese Studie im Gegensatz zum großen, weltweiten ENGAGE-Register bietet, ergibt sich durch den Einschluss von Notfalleingriffen bei rupturierten AAA (5 %) und der Durchführung von operativen Ausbildungseingriffen an einer Universitätsklinik. Bezüglich der technischen und klinischen Erfolgsrate, sowie der Operationsdauer resultierten somit signifikante Unterschiede. Intraoperativ zeigten sich in 24 % ein Endoleak-Typ-II und in jeweils 3 % ein Endoleak-Typ-I bzw. Endoleak-Typ-III. Im Nachbeobachtungszeitraum verkleinerte sich der maximale Aneurysmadurchmesser in 43,9 % der Fälle um mehr als 3 mm und 7,3 % der Patienten dagegen wiesen eine Zunahme des Aneurysmadurchmessers auf. Die Reinterventionsrate lag im Patientenkollektiv bei 13,4 %. Die 30-Tages-Letalitätsrate lag mit 2 % über der des ENGAGE-Registers mit 1,3 %. EVAR mit dem Endurant®-Stentgraft ist bei sorgfältiger Einhaltung der „instruction for use“ auch außerhalb von prospektiven Studien und großen Registern eine sichere und effektive Behandlung. Große Register wie ENGAGE sind wichtig, jedoch nicht in allen Aspekten „real world“. Eine Überprüfung der Ergebnisse im klinischen Alltag ist somit weiterhin erforderlich.
Despite its history of more than one hundred years, the phenomenon of
superconductivity has not lost any of its allure. During that time the concept
and perception of the superconducting state - both from an experimental and
theoretical point of view - has evolved in way that has
triggered increasing interest. What was initially believed to simply be the
disappearance of electrical resistivity, turned out to be a universal and
inevitable result of quantum statistics, characterized by many more
aspects apart from its zero resistivity. The insights of
BCS-theory eventually helped to uncover its deep connection to particle physics
and consequently led to the formulation of the Anderson-Higgs-mechanism. The
very core of this theory is the concept of gauge symmetry (breaking). Within the
framework of condensed-matter theory, gauge invariance is only one of several
symmetry groups which are crucial for the description and classification of
superconducting states. \\
In this thesis, we employ time-reversal, inversion, point group and spin
symmetries to investigate and derive possible Hamiltonians featuring spin-orbit
interaction in two and three spatial dimensions.
In particular, this thesis aims at a generalization of existing numerical
concepts to open up the path to spin-orbit coupled (non)centrosymmetric
superconductors in multi-orbital models.
This is done in a two-fold way: On the one hand, we formulate - based on the
Kohn-Luttinger effect - the perturbative renormalization group in the
weak-coupling limit. On the other hand, we define the spinful flow equations of
the effective action in the framework of functional renormalization, which is
valid for finite interaction strength as well. Both perturbative and functional
renormalization groups produce a low-energy effective (spinful) theory that
eventually gives rise to a particular superconducting state, which is investigated
on the level of the irreducible two-particle vertex. The symbiotic relationship
between both perturbative and functional renormalization can be traced back to
the fact that, while the perturbative renormalization at infinitesimal coupling
is only capable of dealing with the Cooper instability, the functional
renormalization can investigate a plethora of instabilities both in the
particle-particle and particle-hole channels. \\
Time-reversal and inversion are the two key symmetries, which are being used to
discriminate between two scenarios. If both time-reversal and inversion symmetry
are present, the Fermi surface will be two-fold degenerate and characterized by a
pseudospin degree of freedom. In contrast, if inversion symmetry is broken, the
Fermi surface will be spin-split and labeled by helicity. In both cases, we
construct the symmetry allowed states in the particle-particle as well as the
particle-hole channel. The methods presented are formally unified and implemented
in a modern object-oriented reusable and extendable C++ code.
This methodological implementation is employed to one member of both families of
pseudospin and helicity characterized systems. For the pseudospin case, we choose
the intriguing matter of strontium ruthenate, which has been heavily
investigated for already twenty-four years, but still keeps puzzling researchers.
Finally, as the helicity based application, we consider the oxide heterostructure
LaAlO$_{3}$/SrTiO$_{3}$, which became famous for its highly mobile two-
dimensional electron gas and is suspected to host topological superconductivity.
Der Erste Weltkrieg und die Nachkriegszeit: Ein Thema, zu dem man scheinbar schon alles gehört und gelesen hat. Dass dem nicht so ist, zeigen Studierende der Philosophischen Fakultät der Universität Würzburg mit ihrer Ausstellung „Krieg │ Frieden. Der Erste Weltkrieg und die Nachkriegszeit“. Ihre Beiträge behandeln Politik, Kunst und Literatur in den am Krieg beteiligten Ländern.
Nach einer historischen Einführung, die auch die Nachkriegssituation in Würzburg themati-siert, vermitteln Texte von Gottfried Benn einen distanzierten Blick auf das Kampfgeschehen: Der Dichter wirkte während des Kriegs als Arzt hinter der Front und verarbeitete seine Erlebnisse u.a. in den „Rönne-Novellen“. Anhand literarischer Portraits von Schützen aus dem Senegal lässt sich wiederum nachvollziehen, wie sich der Blick auf die aus den Kolonien verpflichteten Soldaten in der französischen Literatur widerspiegelt. Unterschiedliche Reaktionen auf den Ersten Weltkrieg und die Nachkriegszeit werden demgegenüber in der italienischen und deutschen Kunst sichtbar, während die hierzulande aus propagandistischen Gründen eröffneten Kriegsmuseen in der Nachkriegszeit schlossen oder sich der Friedensarbeit widmeten. Stimmen namhafter italienischer Politiker, Journalisten, Romanciers und Dichter bringen zum Ausdruck, wie der „Große Krieg / la Grande Guerra“ Italiens kollektives Gedächtnis prägte. Auch in Großbritannien ist das Gedenken daran viel stärker als hierzulande: So prägen die von dem Gedicht „In Flanders Fields“ inspirierten Mohnblumen als „Remembrance Poppies“ die britische Erinnerungskultur bis in unsere Gegenwart.
Individuell gestaltete Text- und Bildtafeln, eine Hörstation mit Leseecke sowie eine Soundinstallation und Bildprojektion am Eröffnungsabend machen den Ausstellungsbesuch informativ und abwechslungsreich. Die interdisziplinäre Ausstellung wurde von rund 90 Studierenden der Fächer Anglistik, Amerikanistik, Germanistik, Geschichtswissenschaft, Kunstgeschichte, Museologie (Ausstellungsmanagement), Romanistik und Slavistik im Wintersemester 2018/19.
Damit die Knochenregeneration lege artis abläuft ist ein sensibles und komplexes Zusammenspiel einer Reihe von Faktoren notwendig. Neben bestimmten Zelltypen, die für die Knochenregeneration essentiell sind, sind auch eine Reihe von Wachstumsfaktoren notwendig um die Kommunikation zwischen den Zellverbänden zu gewährleisten und die einzelnen Entwicklungsstadien zu steuern und zu regulieren.
Zur Untersuchung der Möglichkeit, sowohl die Osteokonduktion als auch Vaskularisation eines Scaffolds zu initiieren, wurden in der vorliegenden Arbeit verschiedene Untersuchungsmethoden eingesetzt. Damit wurden adulte humane mesenchymale Stammzellen untersucht, die zur Differenzierung mit den Wachstumsfaktoren bone morphogenetic protein 2 (BMP 2) und/oder vascular endothelial grwoth factor (VEGF) inkubiert und auf Glas- oder Bruschitoberflächen kultiviert wurden. Die Experimente wurden mittels immunologischer Methoden wie Immunfluoreszenz (IF) und Westernblot (WB), sowie über Rasterelektronenmikroskopie (REM) analysiert. Es konnte mit diesen Methoden gezeigt werden, dass die humanen mesenchymalen Stammzellen (hMSC) auf den verschiedenen Substraten adhärierten und proliferierten. Darüber hinaus konnte in der Arbeit nachgewiesen werden, dass unter diesen bestimmten Bedingungen sowohl knöcherne als auch vaskuläre Zellbildung angeregt werden kann. So konnte sowohl auf Glas als auch auf Bruschit mittels IF und REM zum Teil aus hMSC differenzierte Osteoblasten detektiert werden. Diese zeigten die für Osteoblasten typischen Zellfortsätze, mit denen die Osteoblasten mit den Nachbarzellen in Kontakt stehen. Die beginnende Differenzierung zu Osteoblasten bzw. Endothelzellen konnte auch durch Detektion spezfischer Marker, wie z.B. alkalische Phosphatase und PECAM mittels WB gezeigt werden. Jedoch war die in dieser Arbeit zur Untersuchung der ortsgerichteten Differenzierung auf Bruschitsubstrat eingesetzte Methodik nicht geeignet, eindeutige Aussagen zu treffen. Daher müssen zur Untersuchung dieses Vorganges alternative Methoden entwickelt und optimiert werden.
Bei der vorliegenden Arbeit handelt es sich um eine reine in vitro Studie. Dennoch könnten diese Ergebnisse Hinweise auf das Verhalten von hMSC unter Stimulation mit osteogenen und endothelialen Wachstumsfaktoren in vivo im Tierversuch oder im Menschen liefern.
Allerdings wird es bei der Übertragung auf eine kombiniertes in vitro- in vivo Vorgehen hinauslaufen, da das ungerichtete Wachstum von Gewebsformationen eine Hürde für in vivo Studien darstellt.
Im Rahmen der Präparation und Lagerung von TKs entstehen bei Thrombozyten morphologische, funktionelle und hämostatische Defizite, die unter dem Begriff storage lesion zusammengefasst werden. In dieser Dissertation wurde untersucht, ob durch Zugabe des kurzzeitig und reversibel wirksamen NO-Donors DEA/NO zu Apherese-TKs Zeichen der storage lesion über eine 5-tägige Lagerung vermindert werden können. Dafür wurde den Apherese-TKs direkt nach der Herstellung 5 nM DEA/NO zugesetzt. An den Tagen 0 (nach Abklingen der NO-Donor-Wirkung), 2 und 5 wurden verschiedene funktionelle Systeme der Thrombozyten analysiert. Verglichen mit früheren Untersuchungen von unbehandelten TKs ergab sich unter Einsatz von DEA/NO eine abgeschwächte Aktivierung der inhibitorischen Signalwege mit geringerem Anstieg der VASP-Phosphorylierung und des cGMP-Spiegels sowie mit stabilem PDE5A-Gehalt. Gemessen anhand der P-Selektin-Expression und der Fibrinogenbindung zeigte sich ein unverändert niedriger Präaktivierungsgrad der Thrombozyten bei erhaltener Stimulierbarkeit. Bei der Oberflächenexpression von purinergen Rezeptoren war der Rückgang der stimulierten Mobilisation während der 5-tägigen Lagerung im Vergleich zu unbehandelten TKs vermindert. Damit war unter dem Einfluss von DEA/NO eine Abschwächung von Phänomenen der storage lesion zu beobachten. Für eine mögliche klinische Anwendung des NO-Donors DEA/NO bei der TK-Herstellung sind allerdings weitere Studien bezüglich Wirksamkeit und möglicher unerwünschter Wirkungen in vivo notwendig. Darüber hinaus muss eine technische Lösung für die sterile Zugabe von DEA/NO gefunden werden.
An efficient foraging strategy is one of the most important traits for the fitness of animals. The theory of optimal foraging tries to predict foraging behaviour through the overarching question: how animals should forage so as to minimize costs while maximizing profits? Social insects, having occupied nearly every natural niche through widely different strategies, offer themselves as an ideal group to study how well optimal foraging theory can explain their behaviour and success.
Specialization often leads to unique adaptations in morphology and behaviour. I therefore decided to investigate the behaviour of Megaponera analis. This ponerine ant species is specialized on hunting only termites of the subfamily Macrotermitinae at their foraging sites. Their foraging behaviour is regulated by a handful of individual scouts (10-20) that search for termite foraging sites before returning to the nest to recruit a large number of nestmates (200-500 ants). These ants then follow the scout in a column formation to the termites and after the hunt return together to the nest, these raids occur two to five times per day.
Predators of highly defensive prey likely develop cost reducing adaptations. The evolutionary arms race between termites and ants led to various defensive mechanisms in termites, e.g. a caste specialized in fighting predators. As M. analis incurs high injury/mortality risks when preying on termites, some risk mitigating adaptations have evolved. I show that a unique rescue behaviour in M. analis, consisting of injured nestmates being carried back to the nest, reduces combat mortality. These injured ants “call for help” with pheromones present in their mandibular gland reservoirs. A model accounting for this rescue behaviour identifies the drivers favouring its evolution and estimates that rescuing allows for maintaining a 29% larger colony size. Heavily injured ants that lost too many legs during the fight on the other hand are not helped. Interestingly, this was regulated not by the helper but by the uncooperativeness of the injured ant. I further observed treatment of the injury by nestmates inside the nest through intense allogrooming directly at the wound. Lack of treatment increased mortality from 10% to 80% within 24 hours, with the cause of death most likely being infections.
Collective decision-making is one of the main mechanisms in social insects through which foraging is regulated. However, individual decision-making can also play an important role, depending on the type of foraging behaviour. In M. analis only a handful of individuals (the scouts) hold all the valuable information about foraging sites. I therefore looked at predictions made by optimal foraging theory to better understand the interplay between collective and individual decision-making in this obligate group-raiding predator. I found a clear positive relation between raid size and termite abundance at the foraging site. Furthermore, selectivity of the food source increased with distance. The confirmation of optimal foraging theory suggests that individual scouts must be the main driver behind raid size, choice and raiding behaviour. Therefore most central place foraging behaviours in M. analis were not achieved by collective decisions but rather by individual decisions of scout ants. Thus, 1% of the colony (10–20 scouts) decided the fate and foraging efficiency of the remaining 99%.
Division of labour is one of the main reasons for the success of social insects. Worker polymorphism, age polyethism and work division in more primitive ants, like the ponerines, remain mostly unexplored though. Since M. analis specializes on a defensive prey, adaptations to reduce their foraging costs can be expected. I found that the work division, task allocation and column-formation during the hunt were much more sophisticated than was previously thought. The column-formation was remarkably stable, with the same ants resuming similar positions in subsequent raids and front ants even returning to their positions if displaced in the same raid. Most of the raid tasks were not executed by predetermined members of the raid but were filled out as need arose during the hunt, with a clear preference for larger ants to conduct most tasks.
I show that specialization towards a highly defensive prey can lead to very unique adaptations in the foraging behaviour of a species. I explored experimentally the adaptive value of rescue behaviour focused on injured nestmates in social insects. This was not only limited to selective rescuing of lightly injured individuals by carrying them back (thus reducing predation risk) but moreover includes a differentiated treatment inside the nest. These observations will help to improve our understanding of the evolution of rescue behaviour in animals. I further show that most optimal foraging predictions are fulfilled and regulated by a handful of individuals in M. analis. Lastly, I propose that the continuous allometric size polymorphism in M. analis allows for greater flexibility in task allocation, necessary due to the unpredictability of task requirements in an irregular system such as hunting termites in groups. All of my observations help to further understand how a group-hunting predator should forage so as to minimize costs while maximizing profits.
Das Akute Lungenversagen stellt ein schweres Krankheitsbild dar. Wir führten eine retrospektive Analyse an 25 Patienten durch, die einer Extrakorporalen Membranoxygenierung aufgrund eines akuten Lungenversagens zugeführt wurden. Allgemeine und ARDS-spezifische Kriterien und die Schwere des ARDS schienen keinen Einfluss auf einen Therapieerfolg zu haben. Die Mehrzahl der nicht-überlebenden Patienten verstarb an einem Multiorganversagen und nicht an einer Hypoxämie.
Knapp über 6 Mio. HIV-Infizierte leben in Südafrika, darunter ca. 410.000 Kin-der unter 15 Jahren. Somit ist es das am meisten von HIV betroffene Land weltweit. Seit 2004 haben die Infizierten, initiiert durch das staatliche Roll-Out Programm, kostenfrei Zugang zur HAART. Aufgrund der begrenzten Ressour-cen sind die Therapieschemata und das Patientenmonitoring eingeschränkt.
Im pädiatrischen Patientenkollektiv beinhaltete die Erstlinien-Therapie zum Zeitpunkt dieser Untersuchung Stavudin und Azidothymidin - beides ältere Medikamente, die eng mit der Entwicklung eines Lipodystrophie-Syndroms verknüpft werden. Das Lipodystrophie-Syndrom fasst verschiedene Fettumverteilungsstörungen (die Lipoatrophie und Lipohypertrophie) sowie Fettstoffwechselstörungen zusammen. Neben erheblichen Auswirkungen auf die Compliance der Patienten fürchtet man frühzeitig kardiovaskuläre Komplikationen. Die geschätzte Prävalenz dieses Syndroms bei HIV-positiven Kindern beträgt circa 30% und erlangt so besondere Relevanz.
Aufgrund der mangelnden Reversibilität der Fettumverteilung ist eine frühzeitige Diagnose, gefolgt von einer therapeutischen Maßnahme entscheidend. Die Diagnose des Lipodystrophie-Syndroms erfolgt in der Regel durch einen pädiatrischen HIV-Spezialisten; an apparativen Untersuchungsmethoden stehen DEXA-Scans und Einzelschicht-CT-Aufnahmen zur Verfügung. In Südafrika können diese Untersuchungen, außerhalb von Forschungseinrichtungen, aufgrund der begrenzten finanziellen Mittel nicht durchgeführt werden und auch die Anzahl der pädiatrischen HIV-Spezialisten ist begrenzt. Daher ist es von großer Bedeutung, ein einfaches Screening-Werkzeug zu entwickeln, um das Lipodystrophie-Syndrom auch in Ressourcen-armen Gebieten frühzeitig diag-nostizieren zu können.
In der vorliegenden Arbeit wurde, gerade im Hinblick auf die Entwicklung eines einfachen Lipodystrophie-Syndrom-Screening-Werkzeuges, die Vergleichbarkeit anthropometrischer Messungen bestimmt. Daneben wurden Risikofaktorenanalysen durchgeführt.
Insgesamt erfolgten hierzu an 72 HIV-positiven Kindern ausführliche anthropometrische Messungen. Die Messungen wurden jeweils von einer professionellen Ernährungsberaterin und einer eigens eingearbeiteten Medizinstudentin erfasst. Die Vergleichbarkeit wurde anhand eines gepaarten t-Testes und Intraklassenkorrelationen ermittelt. Zusätzlich erfolgte eine Datenerhebung aus den vorliegenden Patientenakten.
Die als Screening-Methode favorisierten Hautfaltendicken-Messungen zur Ermittlung des subkutanen Fettgewebes, zeigten im t-Test, bis auf die Bestim-mung der Hautfaltendicke des Abdomens und Oberschenkels, signifikante Unterschiede. Die Intraklassenkorrelation konnte jedoch für alle Messwerte eine gute bis sehr gute Übereinstimmung finden. Unter Verwendung der hier vorgestellten Daten konnten wir in einer Folgestudie zeigen, dass sich die Hautfaltendicke-Messung im Bereich des Bizeps trotzdem zur frühzeitigen Detektion einer Lipodystrophie eignen könnte, da der Einfluss der hier aufgezeigten Unterschiede nur gering ist [99]. Diese Erkenntnis hat große Relevanz und lässt hoffen, dass in Zukunft auch bei Patienten in ländlichen Gebieten Südafrikas die Diagnose eines Lipodystrophie-Syndroms frühzeitig gestellt und mit einem Medikamentenwechsel reagiert werden kann.
Anhand der z-Scores der Taillen-zu-Hüft (WHR) Verhältnisse und der Brust-zu-Arm (TAR) Verhältnisse erfolgte die Einteilung der Studienteilnehmer in Lipodystrophie-positiv und Lipodystrohie-negativ. Kleine negative Zusammen-hänge ergaben sich zwischen dem Krankheitsstadium sowie der Baseline CD4-Zellzahl und den Merkmalen einer Lipodystrophie. Ähnliche Ergebnisse sind auch in anderen Studien zu finden. Man geht davon aus, dass ein niedrigeres Krankheitsstadium mit einer besseren Compliance und somit mit einem höheren Risiko zur Entwicklung eines Lipodystrophie-Syndroms verbunden ist. Weiterhin scheint sich eine bessere immunologische Ausgangssituation protektiv auf die Entwicklung eines Lipodystrophie-Syndroms auszuwirken. Eine weitere Korrelation ergab sich außerdem zwischen der LPV/r Dosierung und dem WHR-z-Score, welches den bereits bekannten Zusammenhang zwischen der Einnahme von Proteaseinhibitoren mit der Entwicklung einer Lipohypertrophie stützt. Anhand von Kreuztabellen konnte weder für Stavudin, noch für Azidothymidin ein eindeutiger Hinweis für ein erhöhtes Risiko in der Entwicklung eines Lipodystrophie-Syndroms gefunden werden.
Die Kriterien zur Einteilung in Lipodystrophie-positiv und Lipodystrophie-negativ wurden aus zwei unabhängigen Studien übernommen und erlauben für sich genommen nicht die Diagnose einer Lipodystrophie. Die anhand dieser Einteilung vorgenommene Risikofaktorenanalyse ist deswegen nur bedingt aussagefähig, hinzu kommen die geringe Fallzahl und die heterogene Patientengruppe dieser Studie. Aufgrund der Kombinationstherapie aus mindestens drei antiretroviralen Medikamenten erschwert auch dies die Charakterisierung der Risikofaktoren für die Entwicklung eines Lipodystrophie-Syndroms. Diese ist jedoch, neben der frühen Diagnose, entscheidend um präventive Maßnahmen ergreifen zu können.
Trotz der vorhandenen Limitationen dieser Studie konnten Risikofaktoren identifiziert werden, welche sich auch in der Literaturrecherche bestätigen ließen. Aufgrund verschiedener Berichte über ein ungünstiges Nebenwirkunsgprofil, mit unter anderem hohem Risiko für die Entwicklung eines Lipodystrophie-Syndroms, wurde das Medikament Stavudin aus den südafrikanischen Therapieleitlinien herausgenommen. Weiterhin konnte eine ausreichende Verlässlichkeit der anthropometrischen Messungen gezeigt werden, welche entscheidend für die Entwicklung eines einfachen LDS-Screening-Werkzeugs ist. Diese Daten wurden 2013 im „Pediatric Infectious Disease Journal“ veröffentlicht.
Für die Zukunft wären größere Studien mit besseren diagnostischen Möglich-keiten wünschenswert, um das Lipodystrophie-Syndrom bestmöglich zu charakterisieren und Risikofaktoren zu benennen. Gerade für das pädiatrische Patientenkollektiv scheint es besonders im Hinblick auf die lebenslange Therapiedauer wichtig Medikamentennebenwirkungen frühzeitig zu erkennen, zu reduzieren und Alternativmedikamente bereitzustellen. Für ressourcenarme Länder ist dies eine besondere Herausforderung. Hier stehen nur begrenzt Ausweichmedikamente zur Verfügung.
Economists (should) care about regions! On the one hand this is true because macroeconomic shocks have vastly different effects across regions. The pressing topics of robotization
and artificial intelligence, Brexit, or U.S. tariffs will affect Würzburg differently than Berlin,
implying varying interests among its population, firms and politicians. On the other hand,
shocks in individual regions, such as inventions, bankruptcies or the attraction of a major
plant can, through trade and input-output linkages, magnify to aggregate effects of macroe-
conomic importance. Yet, regional heterogeneities in Germany and the complicated network
of linkages that connect regions are still not well documented nor understood. A fact that
is especially true for local labor markets that are of core interest to regional policy makers
and that also feature substantial heterogeneity.
This thesis provides a thorough quantification of such heterogeneities and an in-depth analysis of the sources and mechanisms that drive these differences.
Nationalparks sind das älteste und bekannteste flächenbezogene Naturschutzinstrument weltweit. Für den Erhalt einer nachhaltigen Lebensgrundlage und die Entwicklung der Biodiversität sowie für mehr Naturdynamik in der Landschaft haben sie eine sehr große Bedeutung, auch in unseren Breiten. Dennoch ist die Einstellung zu Nationalparks von Seiten der unmittelbaren Anwohner nicht immer unproblematisch. Entsprechend versucht die vorliegende wissenschaftliche Analyse neue Erkenntnisse bezüglich der Akzeptanz der Nationalparks Bayerischer Wald und Berchtesgaden, den ältesten Deutschlands, aufzuzeigen. Empirische Grundlagen für diese Studie sind eine bayernweite Online-Befragung, qualitative Experteninterviews und aufwändige repräsentative schriftliche Befragungen in den Nationalpark-Landkreisen Regen und Freyung-Grafenau bzw. Berchtesgadener Land im Jahr 2018. Auch die zeitliche Entwicklung der Akzeptanz wird auf Basis der Ergebnisse von Vorgängerstudien, soweit möglich, berücksichtigt. Dabei sind es ökonomische, emotionale, interpersonelle, soziokulturelle und nicht zuletzt für Geographen besonders interessante raumzeitliche Prädiktoren der Akzeptanz beider Nationalparks, die im Fokus der Untersuchungen stehen.
Die vorliegende Doktorarbeit widmet sich als Beitrag sowohl zur Körper- bzw. Geschlechtergeschichte als auch zur Selbstzeugnisforschung autobiographischen Texten aus den 70er bis 90er Jahren des 20. Jahrhunderts, in denen gynäkologische Krebserkrankungen eine zentrale Rolle einnahmen. Dabei liegt der Schwerpunkt auf dem Vergleich von Tagebüchern dreier Frauen aus dem Deutschen Tagebucharchiv in Emmendingen – einer Ärztin, einer Lehrerin und einer Sozialpädagogin- sowie publizierten Biographien Hildegard Knefs, Maxie Wanders, Ruth Picardies und Chilly Ants in Hinblick auf Gemeinsamkeiten und Unterschiede in Aufbau, Sprache und vor allem behandelte Themen.
Dies erfolgt zunächst unter Behandlung des historischen Kontextes durch Vermittlung medizinischer Hintergründe anhand von Fachbüchern aus dem Zeitraum, politischer Umstände mit dem Schwerpunkt auf die Frauengesundheitsbewegung sowie zuletzt der gesellschaftlich-öffentlichen Situation mit dem Kernpunkt der Tabuisierung des Themas Krebs. Hieran schließt sich der Vergleich der Texte im Bezug auf verschiedene Themen an mit den zentralen Aspekten: zum Einen die Kranke bzw. Patientin als soziale Figur und ihre Beziehung zur sozialen Welt, zum Anderen ihre Sicht auf die Krankheit mit den begleitenden Themen Krankheitsbewältigung, Krankheitsgewinn sowie Gedanken im Bezug auf Sterben und Tod. Hierbei wird auch Bezug genommen auf den Einfluss der Frauengesundheitsbewegung in der Darstellung und Gewichtung der einzelnen Themen. Zuletzt erfolgt eine zusammenfassende Schlussanalyse.
Woodhouse-Sakati syndrome (WSS) is a rare multisystemic, autosomal recessive disease. The underlying cause of WSS are mutations of C2orf37 gene, which result in a truncated protein. Little is known about the function of C2orf37 (DDB1-CUL4A-associated factor 17, DCAF17) apart from it being part of the DDB1-CUL4-ROC1 E3 ubiquitin ligase complex, specifically binding directly to DDB1 and serving as a substrate recruiter for E3. There are two major isoforms of DCAF17: beta (65 kDa, 520 amino acids) and alpha (27 kDa, 240 amino acids), which is a C-terminal part of beta. The intracellular localization of the WSS protein is thought to be primarily the nucleolus. A murine ortholog protein was found to be expressed in all tissues with a relatively higher expression in the brain, liver, and skin.The aim of this work was to investigate DCAF17 in HeLa cells in more detail, in particular the redistribution of both WSS isoforms on the subcellular and -nuclear level as well as their chemical features. For these experiments, I developed, through recombinant expression and affinity purification, a specific polyclonal antibody against a WSS-epitope 493-520. Furthermore, three other specific polyclonal antibodies were obtained through affinity purification with help of commercially produced high-affinity epitope peptides.By means of these antibodies, I determined- through immunofluorescence and subcellular protein fractionation- that, apart from the redistribution of the WSS protein within the non-soluble = chromatin-bound nuclear fraction, a significant amount of both WSS isoforms is present in the soluble nuclear fraction. Indeed, treatment of purified nuclear envelopes with an increasing concentration of NaCl as well as urea confirmed a non-covalent binding of the WSS protein to the nuclear envelope with the detachment ofbeta-WSS at a lower NaCl concentration than alpha-WSS. In regard to the chromatin-bound WSS protein, I performed hydrolysis of nuclear and nucleolar extract with DNase and RNase. The results indicate that the WSS protein is bound to DNA but not RNA, with alpha-WSS being possibly located more abundantly in the nucleolus, whereas beta-WSS within other subnuclear departments. Furthermore, in all the above-mentioned experiments, a presence of an 80-kDa protein, which specifically reacted with the polyclonal high-affinity antibodies and showed similar redistribution and chemical features as alpha- and beta-WSS, was observed. In order to investigate whether this protein is a posttranslationally modified WSS isoform, I performed deglycosylation and dephosphorylation of nuclear extract, which showed no disappearance or change in abundance of the 80-kDa band on Western blot. While other ways of poststranslational modification cannot be excluded as the cause of occurrence of the 80-kDa protein, an existence of a third, yet undescribed, major isoform is also conceivable. Summarizing, this work contributed to a deeper characterization of the WSS protein, which can help future investigators in developing new experimental ideas to better understand the pathology of WSS.
Tobramycin und Colistin sind zwei Standardantibiotika bei der inhalativen Behandlung von Patienten mit zystischer Fibrose (CF), die chronisch mit Pseudomonas aeruginosa besiedelt sind. In dieser Arbeit wurde die Resistenzentwicklung von Pseudomonas aeruginosa gegen Tobramycin und Colistin bei 1844 Isolaten beobachtet. Die Pseudomonas-aeruginosa-Isolate wurden von 22 Patienten mit CF gewonnen, die eine alternierende Inhalationstherapie mit Tobramycin und Colistin erhalten hatten. Eine Tobramycinresistenz wurde bei 30,4% der Isolate und bei 72,7% der Patienten beobachtet. Im Gegensatz hierzu waren alle Isolate sensibel gegenüber Colistin, und es entwickelte auch kein Patient eine Colistin-Resistenz. In molekulargenetischen Analysen ausgewählter Pseudomonas-aeruginosa-Isolate hatte es den Anschein, dass die Patienten im Verlauf der Erkrankung jeweils nur mit einem Genotyp besiedelt waren. Zusammenfassend kann gesagt werden, dass eine Resistenzentwicklung gegen Tobramycin unter Inhalationstherapie stattfindet, während eine Resistenzentwicklung gegen Colistin die Ausnahme zu bleiben scheint.
Mammalian haloacid dehalogenase (HAD)-type phosphatases are a large and ubiquitous family of at least 40 human members. Many of them have important physiological functions, such as the regulation of intermediary metabolism and the modulation of enzyme activities, yet they are also linked to diseases such as cardiovascular or metabolic disorders and cancer.
Still, most of the mammalian HAD phosphatases remain functionally uncharacterized.
This thesis reveals novel cell biological and physiological functions of the phosphoglycolate phosphatase PGP, also referred to as AUM. To this end, PGP was functionally characterized by performing analyses using purified recombinant proteins to investigate potential protein substrates of PGP, cell biological studies using the spermatogonial cell line GC1, primary mouse lung endothelial cells and lymphocytes, and a range of biochemical techniques to characterize Pgp-deficient mouse embryos.
To characterize the cell biological functions of PGP, its role downstream of RTK- and integrin signaling in the regulation of cell migration was investigated. It was shown that PGP inactivation elevates integrin- and RTK-induced circular dorsal ruffle (CDR) formation, cell spreading and cell migration. Furthermore, PGP was identified as a negative regulator of directed lymphocyte migration upon integrin- and GPCR activation.
The underlying mechanisms were analyzed further. It was demonstrated that PGP regulates CDR formation and cell migration in a PLC- and PKC-dependent manner, and that Src family kinase activities are required for the observed cellular effects. Upon integrin- and RTK activation, phosphorylation levels of tyrosine residues 1068 and 1173 of the EGF receptor were elevated and PLCγ1 was hyper-activated in PGP-deficient cells. Additionally, PGP-inactivated lymphocytes displayed elevated PKC activity, and PKC-mediated cytoskeletal remodeling was accelerated upon loss of PGP activity. Untargeted lipidomic analyses revealed that the membrane lipid phosphatidylserine (PS) was highly upregulated in PGP-depleted cells.
These data are consistent with the hypothesis that the accumulation of PS in the plasma membrane leads to a pre-assembly of signaling molecules such as PLCγ1 or PKCs that couple the activation of integrins, EGF receptors and GPCRs to accelerated cytoskeletal remodeling.
Thus, this thesis shows that PGP can affect cell spreading and cell migration by acting as a PG-directed phosphatase.
To understand the physiological functions of PGP, conditionally PGP-inactivated mice were analyzed. Whole-body PGP inactivation led to an intrauterine growth defect with developmental delay after E8.5, resulting in a gradual deterioration and death of PgpDN/DN embryos between E9.5 and E11.5. However, embryonic lethality upon whole-body PGP inactivation was not caused by a primary defect of the (cardio-) vascular system. Rather, PGP inactivated embryos died during the intrauterine transition from hypoxic to normoxic conditions.
Therefore, the potential impact of oxygen on PGP-dependent cell proliferation was investigated. Analyses of mouse embryonic fibroblasts (MEFs) generated from E8.5 embryos and GC1 cells cultured under normoxic and hypoxic conditions revealed that normoxia (~20% O2) causes a proliferation defect in PGP-inactivated cells, which can be rescued under
hypoxic (~1% O2) conditions. Mechanistically, it was found that the activity of triosephosphate isomerase (TPI), an enzyme previously described to be inhibited by phosphoglycolate (PG) in vitro, was attenuated in PGP-inactivated cells and embryos. TPI constitutes a critical branch point between carbohydrate- and lipid metabolism because it catalyzes the isomerization of the glycolytic intermediates dihydroxyacetone phosphate (DHAP, a precursor of the glycerol backbone required for triglyceride biosynthesis) and glyceraldehyde 3’-phosphate (GADP).
Attenuation of TPI activity, likely explains the observed elevation of glycerol 3-phosphate levels and the increased TG biosynthesis (lipogenesis). Analyses of ATP levels and oxygen consumption rates (OCR) showed that mitochondrial respiration rates and ATP production were elevated in PGP-deficient cells in a lipolysis-dependent manner. However under hypoxic conditions (which corrected the impaired proliferation of PGP-inactivated cells), OCR and ATP production was indistinguishable between PGP-deficient and PGP-proficient cells. We therefore propose that the inhibition of TPI activity by PG accumulation due to loss of PGP activity shifts cellular bioenergetics from a pro-proliferative, glycolytic metabolism to a lipogenetic/lipolytic metabolism.
Taken together, PGP acts as a metabolic phosphatase involved in the regulation of cell migration, cell proliferation and cellular bioenergetics. This thesis constitutes the basis for further studies of the interfaces between these processes, and also suggests functions of PGP for glucose and lipid metabolism in the adult organism.
The aim of the work was the development of thiol-ene cross-linked hydrogels based on functionalized poly(glycidol)s (PG) and hyaluronic acid (HA) for extrusion based 3D bioprinting. Additionally, the functionalization of the synthesized PG with peptides and the suitability of these polymers for physically cross-linked gels were investigated, in a proof of principle study in order to demonstrate the versatile use of PG polymers in hydrogel development.
First, the precursor polymers of the different hydrogel systems were synthesized. For thiol-ene cross-linked hydogels, linear allyl-functionalized PG (P(AGE-co-G)) and three different thiol-(SH-)functionalized polymers, ester-containing PG-SH (PG SHec), ester-free PG-SH (PG-SHef) and HA-SH were synthesized and analysed, The degree of functionalization of these polymers was adjustable.
For physically cross-linked hydrogels, peptide-functionalized PG (P(peptide-co-G)), was synthesized through polymer analogue thiol-ene modification of P(AGE-co-G).
Subsequently, thiol-ene cross-linked hydrogels were prepared with the synthesized thiol- and allyl-functionalized polymers. Depending on the origin of the used polymers, two different systems were obtained: on the one hand synthetic hydrogels consisting of PG-SHec/ef and P(AGE-co-G) and on the other hand hybrid gels, consisting of HA-SH and P(AGE-co-G). In synthetic gels, the degradability of the gels was determined by the applied PG-SH. The use of PG-SHec resulted in hydrolytically degradable hydrogels, whereas the cross-linking with PG-SHef resulted in non-degradable gels.
The physical properties of these different hydrogel systems were determined by swelling, mechanical and diffusion studies and subsequently compared among each other. In swelling studies the differences of degradable and non-degradable synthetic hydrogels as well as the differences of synthetic compared to hybrid hydrogels were demonstrated.
Next, the stiffness and the swelling ratios (SR) of the established hydrogel systems were examined in dependency of different parameters, such as incubation time, polymer concentration and UV irradiation. In general, these measurements revealed the same trends for synthetic and hybrid hydrogels: an increased polymer concentration as well as prolonged UV irradiation led to an increased network density. Moreover, it was demonstrated that the incorporation of additional non-bound HMW HA hampered the hydrogel cross-linking resulting in gels with decreased stiffness and increased SR. This effect was strongly dependent on the amount of additional HMW HA.
The diffusion of different molecular weight fluorescein isothiocyanate-dextran (FITC-dextran) through hybrid hydrogels (with/without HMW HA) gave information about the mesh size of these gels. The smallest FITC-dextran (4 kDa) completely diffused through both hydrogel systems within the first week, whereas only 55 % of 40 kDa and 5-10 % HMW FITC-dextrans (500 kDa and 2 MDa) could diffuse through the networks.
The applicability of synthetic and hybrid hydrogels for cartilage regeneration purpose was investigated through by biological examinations. It was proven that both gels support the survival of embedded human mesenchymal stromal cells (hMSCs) (21/28 d in vitro culture), however, the chondrogenic differentiation was significantly improved in hybrid hydrogels compared to synthetic gels. The addition of non-bound HMW HA resulted in a slightly less distinct chondrogenesis.
Lastly the printability of the established hydrogel systems was examined. Therefore, the viscoelastic properties of the hydrogel solutions were adjusted by incorporation of non-bound HMW HA. Both systems could be successfully printed with high resolution and high shape fidelity.
The introduction of the double printing approach with reinforcing PCL allowed printing of hydrogel solutions with lower viscosities. As a consequence, the amount of additional HMW HA necessary for printing could be reduced allowing successful printing of hybrid hydrogel solutions with embedded cells. It was demonstrated that the integrated cells survived the printing process with high viability measured after 21 d. Moreover, by this reinforcing technique, robust hydrogel-containing constructs were fabricated.
In addition to thiol-ene cross-linked hydrogels, hydrogel cross-linking via ionic interactions was investigated with a hybrid hydrogel based on HMW HA and peptide-functionalized PG. Rheological measurements revealed an increase in the viscosity of a 2 wt.% HMW HA solution by the addition of peptide-functionalized PG. The increase in viscosity could be attributed to the ionic interactions between the positively charge PG and the negatively charge HMW HA.
In conclusion, throughout this thesis thiol-ene chemistry and PG were introduced as promising cross-linking reaction and polymer precursor for the field of biofabrication. Furthermore, the differences of hybrid and synthetic hydrogels as well as chemically and physically cross-linked hydrogels were demonstrated.
Moreover, the double printing approach was demonstrated to be a promising tool for the fabrication of robust hydrogel-containing constructs. It opens the possibility of printing hydrogels that were not printable yet, due to too low viscosities.
The Software Defined Networking (SDN) paradigm offers network operators numerous improvements in terms of flexibility, scalability, as well as cost efficiency and vendor independence. However, in order to maximize the benefit from these features, several new challenges in areas such as management and orchestration need to be addressed. This dissertation makes contributions towards three key topics from these areas.
Firstly, we design, implement, and evaluate two multi-objective heuristics for the SDN controller placement problem. Secondly, we develop and apply mechanisms for automated decision making based on the Pareto frontiers that are returned by the multi-objective optimizers. Finally, we investigate and quantify the performance benefits for the SDN control plane that can be achieved by integrating information from external entities such as Network Management Systems (NMSs) into the control loop. Our evaluation results demonstrate the impact of optimizing various parameters of softwarized networks at different levels and are used to derive guidelines for an efficient operation.
Nanodiamant bietet in der Medizin und in der Biologie zahlreiche Anwendungsmöglichkeiten aufgrund der guten Biokompatibilität und geringen Toxizität. Durch die umfangreichen Funktionalisierungsmöglichkeiten der Oberfläche der nanometergroßen Partikel können viele unterschiedliche Wirkstoffe, Rezeptormoleküle oder Peptidsequenzen angebunden werden ,die zusammen mit Nanodiamant ein anderes, durchaus besseres Wirkprofil aufweisen als der Wirkstoff allein. Ziel dieser Arbeit war die Synthese eines pH-labilen Linkersystems, dass hydroxylhaltige Wirkstoffe kovalent bindet und zusammen mit Nanodiamant in die Zelle, in der ein saurer pH-Wert herrscht, eingeschleust werden. Über die Änderung des pH-Wertes in der Zelle soll der Wirkstoff freigesetzt werden und seine Wirkung entfalten können.
Weiterhin wurde ein pH-labiles Linkersystem auf der Basis eines Hydrazons hergestellt. Über das synthetisierte Hydrazinderivat können Wirkstoffe, die über eine Aldehyd- oder Ketonfunktion verfügen angebunden werden und pH-labil in der Zelle freigesetzt werden. Zusätzlich trägt der Nanodiamant ein kovalent angebundenes Targeting-Molekül, welches eine verbesserte Adressierung der Wirkorte gewähr¬leisten soll. Die Freisetzung wurde mittels UV-Vis-Spektroskopie detektiert und ausgewertet.
Neben der spezifischen Funktionalisierung von Nanodiamant besitzt auch die Interaktion der Nanodiamantpartikel mit biologischen Medien eine besondere Bedeutung für zukünftige biomedizinische Anwendungen. Wenn die Partikeloberfläche durch Proteinadsorption gegenüber dem Wirkort abgeschirmt wird, so kann der angebundene Wirkstoff gegebenenfalls nicht freigesetzt werden und somit nicht seine Wirkung entfalten und bleibt letztlich ungenutzt. So war es von besonderem Interesse die Wechselwirkungen von Nanodiamant in Humanserum und auch weiteren physiologischen Medien zu untersuchen. Dabei wurden sowohl freie Nanodiamantpartikel als auch solche, die auf klinisch bereits eingesetzten Gerüstmaterialien im Bereich der Therapie großer Knochendefekte adsorbiert waren, untersucht. Auch wurden die Wechselwirkungen von Nanodiamant mit der physiologischen Umgebung untersucht, die zur Agglomeration der Nanopartikel führen können. Es wurde ein unter¬schiedliches Agglomerationsverhalten der Nanodiamanten in wässriger Umgebung verglichen mit Nanodiamanten in physiologischen Medien sowie deren Stabilität im Serum beobachtet.
Durch die in dieser Arbeit vorgestellten Untersuchungen konnten wichtige Erkenntnisse zur Wechselwirkung verschieden präparierter und funktionalisierter Nanodiamanten mit physiologisch relevanten Umgebungen sowie zu stimuli-responsiven Wirkstofffreisetzung aus Nanodiamant-Konjugaten gewonnen werden. Zudem wurde mit der Untersuchung der angelagerten Proteine um Nanodiamant ein erster Schritt in Richtung eines umfassenden Verständnisses der Wechselwirkung dieses Materials mit biologischen Umgebungen unternommen. Auch wenn diese Wechselwirkungen sehr komplex sind, so sind erste Aussagen bezüglich der Art der angelagerten Proteine möglich. Erste Versuche der Stabilisierung von Nanodiamant in physiologischen Medien wurden ebenfalls erfolgreich durchgeführt und zeigen eine effiziente und einfache Möglichkeit, Nanodiamant in biologischen Medien vor der Agglomeration zu bewahren. Die im Rahmen dieser Arbeit gewonnen Erkenntnisse bezüglich mehrfacher Funktionalisierungsmöglichkeiten von Nanodiamant sowie dessen Stabilisierung in physiologischen Medien zeigen die breite Anwendungsmöglichkeit und das enorme Potential von Nanodiamant im Bereich medizinischer und biologischer Anwendungen auf.
Neben der Chemotherapie ist heutzutage auch die Hyperthermie-Behandlung eine wichtige Säule der antitumorösen Therapie. Während der sogenannten HIPEC Therapie (Hypertherme intraperitoneale Chemoperfusion) werden die beiden Arten der Therapieformen kombiniert und in der klinischen Praxis erfolgreich angewendet. Genauere Kenntnisse über die zu Grunde liegenden toxikologischen in-vitro Mechanismen könnten zu neuen Möglichkeiten in der klinischen Anwendung führen. In unserer Arbeit untersuchten wir verschiedenen Tumorzelllinien (HT29,CaCo-2,HCT116,HaCaT) in Kombination mit Cisplatin und Hyperthermie mit verschiedenen Methoden, wie zum Beispiel Mikrokerntest, Comet-Assay, Durchflusszytometrie, Vitalitätstest und mikroskopischen Analysen. Unsere Ergebnisse führten uns zu der Hypothese, dass Hyperthermie alleine zu einer sogenannte mitotic catastrophe führt und zum Absterben der Tumorzellen. Im Gegensatz dazu zeigten Tumorzellen, welche mit Cisplatin alleine oder auch in Kombination mit Hyperthermie nicht in die Mitose eintreten und daher nicht durch Apoptose in den Zelltod gehen.
Functionalization of cells, extracellular matrix components and proteins for therapeutic application

Glycosylation is a biochemical process leading to the formation of glycoconjugates by linking glycans (carbohydrates) to proteins, lipids and various small molecules. The glycans are formed by one or more monosaccharides that are covalently attached, thus offering a broad variety depending on their composition, site of glycan linkage, length and ramification. This special nature provides an exceptional and fine tunable possibility in fields of information transfer, recognition, stability and pharmacokinetic. Due to their intra- and extracellular omnipresence, glycans fulfill an essential role in the regulation of different endogenous processes (e.g. hormone action, immune surveillance, inflammatory response) and act as a key element for maintenance of homeostasis. The strategy of metabolic glycoengineering enables the integration of structural similar but chemically modified monosaccharide building blocks into the natural given glycosylation pathways, thereby anchoring them in the carbohydrate architecture of de novo synthesized glycoconjugates. The available unnatural sugar molecules which are similar to endogenous sugar molecules show minimal perturbation in cell function and - based on their multitude functional groups - offer the potential of side directed coupling with a target substance/structure as well as the development of new biological properties. The chemical-enzymatic strategy of glycoengineering provides a valuable complement to genetic approaches.
This thesis primarily focuses on potential fields of application for glycoengineering and its further use in clinic and research. The last section of this work outlines a genetic approach, using special Escherichia coli systems, to integrate chemically tunable amino acids into the biosynthetic pathway of proteins, enabling specific and site-directed coupling with target substances. With the genetic information of the methanogen archaea, Methanosarcina barkeri, the E. coli. system is able to insert a further amino acid, the pyrrolysine, at the ribosomal site during translation of the protein. The natural stop-codon UAG (amber codon) is used for this newly obtained proteinogenic amino acid.
Chapter I describes two systems for the integration of chemically tunable monosaccharides and presents methods for characterizing these systems. Moreover, it gives a general overview of the structure as well as intended use of glycans and illustrates different glycosylation pathways. Furthermore, the strategy of metabolic glycoengineering is demonstrated. In this context, the structure of basic building blocks and the epimerization of monosaccharides during their metabolic fate are discussed.
Chapter II translates the concept of metabolic glycoengineering to the extracellular network produced by fibroblasts. The incorporation of chemically modified sugar components in the matrix provides an innovative, elegant and biocompatible method for site-directed coupling of target substances. Resident cells, which are involved in the de novo synthesis of matrices, as well as isolated matrices were characterized and compared to unmodified resident cells and matrices. The natural capacity of the matrix can be extended by metabolic glycoengineering and enables the selective immobilization of a variety of therapeutic substances by combining enzymatic and bioorthogonal reaction strategies. This approach expands the natural ability of extracellular matrix (ECM), like the storage of specific growth factors and the recruitment of surface receptors along with synergistic effects of bound substances. By the selection of the cell type, the production of a wide range of different matrices is possible.
Chapter III focuses on the target-oriented modification of cell surface membranes of living fibroblast and human embryonic kidney cells. Chemically modified monosaccharides are inserted by means of metabolic glycoengineering and are then presented on the cell surface. These monosaccharides can later be covalently coupled, by “strain promoted azide-alkyne cycloaddition“ (SPAAC) and/or “copper(I)-catalyzed azide-alkyne cycloaddition“ (CuAAC), to the target substance. Due to the toxicity of the copper catalysator in the CuAAC, cytotoxicity analyses were conducted to determine the in vivo tolerable range for the use of CuAAC on living cell systems. Finally, the efficacy of both bioorthogonal reactions was compared.
Chapter IV outlines two versatile carrier – spacer – payload delivery systems based on an enzymatic cleavable linker, triggered by disease associated protease. In the selection of carrier systems (i) polyethylene glycol (PEG), a well-studied, Food and Drug Administration approved substance and very common tool to increase the pharmacokinetic properties of therapeutic agents, was chosen as a carrier for non-targeting systems and (ii) Revacept, a human glycoprotein VI antibody, was chosen as a carrier for targeting systems. The protease sensitive cleavable linker was genetically inserted into the N-terminal region of fibroblast growth factor 2 (FGF-2) without jeopardizing protein activity. By exchanging the protease sensitive sequence or the therapeutic payload, both systems represent a promising and adaptable approach for establishing therapeutic systems with bioresponsive release, tailored to pre-existing conditions.
In summary, by site-specific functionalization of various delivery platforms, this thesis establishes an essential cornerstone for promising strategies advancing clinical application. The outlined platforms ensure high flexibility due to exchanging single or multiple elements of the system, individually tailoring them to the respective disease or target site.
In dieser Arbeit wurde ein Verfahren zur effizienten Herstellung von (−)-trans-Cannabidiol (CBD, 10), (−)-trans-Δ9-Tetrahydrocannabinol (Dronabinol, 21) und (−)-trans-Cannabidivarin (CBDV, 30) durch kontinuierliche Synthese untersucht und entwickelt.
CBD konnte durch kontinuierliche Synthese in drei Schritten aus Olivetolcarbonsäuremethylester (OM, 6) und Menthadienol G (3) mit einer Ausbeute von 41 % synthetisiert werden. Bei optimierten Bedingungen betrug die Reinheit nach Kristallisation > 99 %. Die Stereochemie konnte durch Röntgenstrukturanalyse eindeutig als 1R,6R bestimmt werden. Vorteilhaft war dabei, dass Toluol anstatt eines chlorierten Lösungsmittels verwendet werden konnte. Weitere Vorteile waren die kurze Reaktionszeit und die Tatsache, dass die Synthese bei Raumtemperatur durchgeführt werden konnte. Es konnten fünf Nebenprodukte detektiert und identifiziert werden, wovon eines Dronabinol war.
Bei optimierten Reaktionsparametern konnte eine Ausbeute an Dronabinol von 64,5 % erreicht werden. Durch Simulated Moving Bed (SMB)-Chromatographie konnte Dronabinol kontinuierlich mit einem Gehalt von > 95 % hergestellt werden. Nach der Synthese waren vier Verunreinigungen detektierbar, und zwar Olivetol (17), CBD, Exo-Tetrahydrocannabinol (Exo-THC, 23) und Δ8-Tetrahydrocannabinol (Δ8-THC, 22). Durch die SMB-Aufreinigung konnten alle Verunreinigungen auf einen monographiekonformen (USP 37) Gehalt abgereichert werden. Nach der finalen destillativen Aufarbeitung trat eine noch nicht identifizierte Verunreinigung in einem Gehalt von ca. 0,4 Flächen-% auf.
CBDV konnte durch kontinuierliche Synthese in drei Schritten aus Divarincarbonsäuremethylester (DM, 25) und Menthadienol G synthetisiert werden. Die Ausbeute betrug ca. 30 %, die Reinheit nach Kristallisation > 99 %. Es konnten fünf Nebenprodukte detektiert werden, die im Rahmen dieser Arbeit nicht weiter charakterisiert wurden.
Der Syntheseweg bietet durch Modifikation der Seitengruppen an Position 6 (R1) und Position 5 (R2) der Alkylbenzol-Gruppe Zugang zu synthetischen Cannabinoiden mit einem CBD- oder CBDV-Grundgerüst. Es wurden neun neue Cannabinoide hergestellt: 2-Hydroxyethylcannabidiolat (2-HEC, 31), 2-Hydroxypentylcannabidiolat (2 HPC, 32), Glycerylcannabidiolat (GCBD, 33), Cyclohexylcannabidiolat (CHC, 34), Hexylcannabidiolat (HC, 35), N-Methylsulfonylcannabidiolat (NMSC, 36), 2 Hydroxyethylcannabidivarinolat (2-HECBDV, 37), Cyclohexylcannabidivarinolat (CHCBDV, 38) und Hexylcannabidivarinolat (HCBDV, 39).
Die Bindungsaffinität wurde in Cannabinoid-Rezeptor-transfizierten HEK293EBNA-Zellen untersucht, die intrinsische Aktivität in CHO-Zellen, die Induktion von NF-κB (nuclear factor kappa B) sowie von NFAT (nuclear factor of activated T cells) in Jurkat-T Zellen, die Induktion proinflammatorischer Zytokine und Chemokine (Interleukin(IL)-6, IL-1β, CC Chemokinligand 2' (CCL2) und Tumornekrosefaktor(TNF)-α) auf mRNA-Ebene in RAW264.7-Makrophagen und die Expression von proinflammatorischen Zytokinen (IL-1β, IL-6, IL-8, TNF-α) und Prostaglandin E2 (PGE2) auf Proteinebene in primären humanen Monozyten.
Die CBD-Derivate zeigten eine höhere Selektivität für CB2-Rezeptoren. Die CBDV-Derivate HCBDV und CHCBDV zeigten eine spezifische Bindung an CB1- und CB2-Rezeptoren im nanomolaren Bereich. 2-HEC, 2-HPC, GCBD und NMSC wirkten als Agonisten an CB2- und als Antagonisten am CB1-Rezeptor. CHC band an CB1 und CB2 im submikromolaren Bereich und schien ein Agonist für beide Rezeptoren zu sein. 2- HECBD wirkte als Agonist auf CB2-Rezeptoren und als Antagonist auf CB1-Rezeptoren. In Jurkat-T Zellen hemmte NMSC dosisabhängig die Aktivität von NF-κB sowie von NFAT. 2-HEC, 2-HPC und GCBD hemmten die Expression von NFAT ebenfalls dosisabhängig. CHC und HC reduzierten dosisabhängig die Expression von IL-1β- und CCL2-mRNA in RAW264.7-Makrophagen. NMSC hemmte in geringeren Dosen IL-1β, CCL2 sowie TNF-α und induzierte in höheren Dosen einen starken Anstieg der IL-6-mRNA. In primären humanen Monozyten hemmten 2 HEC und GCBD konzentrationsabhängig die Synthese von IL-1β, IL-6 und TNF-α. 2-HPC hemmte dosisabhängig die Bildung von TNF-α und IL-6. HC verminderte dosisabhängig die Freisetzung von TNF-α und IL-6. NMSC steigerte die durch LPS erhöhte Freisetzung von IL-1β noch weiter, hemmte aber TNF-α, IL-8 und PGE2.
Die hier untersuchten CBD- und CBDV-Derivate sind geeignet, gezielt an Cannabinoid-Rezeptoren zu wirken. Einige der Derivate könnten als selektive CB2-Agonisten genutzt werden. Die Länge des aliphatischen Rests an R2 von CBD (Pentyl-Cannabinoiden) und CBDV (Propyl-Cannabinoiden) korrelierte nicht mit der Bindungsaffinität. Eine höhere Polarität an R1 (2-HECBDV > NMSC > GCBD > 2-HEC) schien demgegenüber die agonistische Aktivität an CB2 zu begünstigen. Um den Ergebnissen zur Beziehung zwischen Struktur und Wirkung noch mehr Bedeutung zu geben, wären weitere synthetische Derivate und deren Testung notwendig.
Pro-migratory signals mediated by the tumor microenvironment contribute to the cancer progression cascade, including invasion, metastasis and resistance to therapy. Derived from in vitro studies, isolated molecular steps of cancer invasion programs have been identified but their integration into the tumor microenvironment and suitability as molecular targets remain elusive. The purpose of the study was to visualize central aspects of tumor progression, including proliferation, survival and invasion by real-time intravital microscopy. The specific aims were to monitor the kinetics, mode, adhesion and chemoattraction mechanisms of tumor cell invasion, the involved guidance structures, and the response of invasion zones to anti-cancer therapy. To reach deeper tumor regions by optical imaging with subcellular resolution, near-infrared and infrared excited multiphoton microscopy was combined with a modified dorsal skinfold chamber model. Implanted HT-1080 fibrosarcoma and B16/F10 and MV3 melanoma tumors developed zones of invasive growth consisting of collective invasion strands that retained cell-cell contacts and high mitotic activity while invading at velocities of up to 200 μm per day. Collective invasion occurred predominantly along preexisting tissue structures, including blood and lymph vessels, collagen fibers and muscle strands of the deep dermis, and was thereby insensitive to RNAi based knockdown and/or antibody-based treatment against β1 and β3 integrins, chemokine (SDF-1/CXCL12) and growth factor (EGF) signaling. Therapeutic hypofractionated irradiation induced partial to complete regression of the tumor main mass, yet failed to eradicate the collective invasion strands, suggesting a microenvironmentally privileged niche. Whereas no radiosensitization was achieved by interference with EGFR or doxorubicin, the simultaneous inhibition of β1 and β3 integrins impaired cell proliferation and survival in spontaneously growing tumors and strongly enhanced the radiation response up to complete eradication of both main tumor and invasion strands. In conclusion, collective invasion in vivo is a robust process which follows preexisting tissue structures and is mainly independent of established adhesion and chemoattractant signaling. Due to its altered biological response to irradiation, collective invasion strands represent a microenvironmentally controlled and clinically relevant resistance niche to therapy. Therefore supportive regimens, such as anoikisinduction by anti-integrin therapy, may serve to enhance radio- and chemoefficacy and complement classical treatment regimens.
