The AMADEUS score is not a sufficient predictor for functional outcome after high tibial osteotomy

Purpose
The Area Measurement And Depth Underlying Structures (AMADEUS) classification system has been proposed as a valuable tool for magnetic resonance (MR)-based grading of preoperatively encountered chondral defects of the knee joint. However, the potential relationship of this novel score with clinical data was yet to determine. It was the primary intention of this study to assess the correlative relationship of the AMADEUS with patient reported outcome scores in patients undergoing medial open-wedge high tibial valgus osteotomy (HTO). Furthermore, the arthroscopic ICRS (International Cartilage Repair Society) grade evaluation was tested for correlation with the AMADEUS classification system.
Methods
This retrospective, monocentric study found a total of 70 individuals that were indicated for HTO due to degenerative chondral defects of the medial compartment between 2008 and 2019. A preoperative MR image as well as a pre-osteotomy diagnostic arthroscopy for ICRS grade evaluation was mandatory for all patients. The Knee Osteoarthritis Outcome Score (KOOS) including its five subscale scores (KOOS-ADL, KOOS-QOL, KOOS-Sports, KOOS-Pain, KOOS-Symptoms) was obtained preoperatively and at a mean follow-up of 41.2 ± 26.3 months. Preoperative chondral defects were evaluated using the AMADEUS classification system and the final AMADEUS scores were correlated with the pre- and postoperative KOOS subscale sores. Furthermore, arthroscopic ICRS defect severity was correlated with the AMADEUS classification system.
Results
There was a statistically significant correlation between the AMADEUS BME (bone marrow edema) subscore and the KOOS Symptoms subscore at the preoperative visit (r = 0.25, p = 0.04). No statistically significant monotonic association between the AMADEUS total score and the AMADEUS grade with pre- and postoperative KOOS subscale scores were found. Intraoperatively obtained ICRS grade did reveal a moderate correlative relation with the AMADEUS total score and the AMADEUS grade (r = 0.28, p = 0.02).
Conclusions
The novel AMADEUS classification system largely lacks correlative capacity with patient reported outcome measures in patients undergoing HTO. The MR tomographic appearance of bone marrow edema is the only parameter predictive of the clinical outcome at the preoperative visit.
Purpose
Despite much improved preoperative planning techniques accurate intraoperative assessment of the high tibial valgus osteotomy (HTO) remains challenging and often results in coronal over- and under-corrections as well as unintended changes of the posterior tibial slope. Noyes et al. reported a novel method for accurate intraoperative coronal and sagittal alignment correction based on a three-dimensional mathematical model. This is the first study examining preliminary data via the proposed Noyes approach for accurate intraoperative coronal and sagittal alignment correction during HTO.
Methods
From 2016 to 2020 a total of 24 patients (27 knees) underwent HTO applying the proposed Noyes method (Noyes-Group). Radiographic data was analyzed retrospectively and matched to patients that underwent HTO using the conventional method, i.e., gradual medial opening using a bone spreader under fluoroscopic control (Conventional-Group). All operative procedures were performed by an experienced surgeon at a single orthopaedic university center.
Results
From the preoperative to the postoperative visit no statistically significant changes of the posterior tibial slope were noted in the Noyes-Group compared to a significant increase in the Conventional-Group (p = 0.01). Regarding the axial alignment no significant differences between both groups were observed pre- and postoperatively. The number of over- and under-corrections did not differ significantly between both groups. Linear regression analysis showed a significant correlation of the postoperative medial proximal tibial angle (MPTA) with the position of the weightbearing line on the tibial plateau.
Conclusion
The 3-triangle method by Noyes seems to be a promising approach for preservation of the posterior tibial slope during HTO.
Purpose
The AMADEUS (Area Measurement And DEpth and Underlying Structures) scoring and grading system has been proposed for the MRI based evaluation of untreated focal chondral defects around the knee. The clinical practicability, its correlation with arthroscopically assessed grading systems (ICRS – International Cartilage Repair Society) and thereby its clinical value in terms of decision making and guiding prognosis was yet to determine.
Methods
From 2008 to 2019 a total of 89 individuals were indicated for high tibial valgus osteotomy (HTO) due to tibial varus deformity and concomitant chondral defects of the medial compartment of the knee. All patients received a preoperative MRI (1.5 Tesla or 3.0 Tesla) and pre-osteotomy diagnostic arthroscopy. Chondral defects of the medial compartment were scored and graded with the MRI based AMADEUS by three independent raters and compared to arthroscopic defect grading by the ICRS system. Interrater and intrarater reliability as well as correlation analysis with the ICRS classification system were assessed.
Results
Intraclass correlation coefficients for the various subscores of the AMADEUS showed an overall good to excellent interrater agreement (min: 0.26, max: 0.80). Intrarater agreement turned out to be substantially inferior (min: 0.08, max: 0.53). Spearman correlation revealed an overall moderate correlative association of the AMADEUS subscores with the ICRS classification system, apart from the defect area subscore. Sensitivity of the AMADEUS to accurately identify defect severity according to the ICRS was 0.7 (0.69 for 3.0 Tesla MRI, 0.67 for 1.5 Tesla MRI). The mean AMADEUS grade was 2.60 ± 0.81 and the mean ICRS score 2.90 ± 0.63.
Conclusions
Overall, the AMADEUS with all its subscores shows moderate correlation with the arthroscopic chondral grading system according to ICRS. This suggests that chondral defect grading by means of the MRI based AMADEUS is well capable of influencing and guiding treatment decisions. Interrater reliability shows overall good agreement.
Der zweizeitige Endoprothesenwechsel gilt als Goldstandard der Therapie periprothetischer Gelenkinfektionen. Ein sicherer Ausschluss einer Infektpersistenz ist für den Therapieerfolg essentiel. Es wurde der diagnostische Nutzen synovialer Marker zum Ausschluss einer Infektpersistenz bei einliegendem Kniegelenksspacer untersucht. Weder mikrobiologische noch zytologische Synovia-Analysen sind zum Ausschluss einer Infektpersistenz im Rahmen des zweizeitigen Knieendoprothesenwechsels geeignet.
Purpose. Detection of infection persistence during the two-stage exchange of the knee for periprosthetic joint infection is challenging. Synovial fluid culture (SFC) and synovial white blood cell count (SWBCC) before joint reimplantation are widespread diagnostic means for this indication. The sensitivity and specificity of SFC and of SWBCC for infection persistence before planned reimplantation were evaluated. Methods. 94 two-stage exchanges of the knee with synovial fluid aspiration performed after a drug holiday of at least 14 days and before reimplantation or spacer exchange (planned reimplantation) were retrospectively analyzed. Only cases with at least 3 intraoperative samples at planned reimplantation were included. SFC and SWBCC were compared to pathogen detection (SFC\(_{(culture)}\)/SWBCC\(_{(culture)}\) and to histopathological signs of infection persistence (SFC\(_{(histo)}\)/SWBCC\(_{(histo)}\) from intraoperative samples at planned reimplantation. For SFC, the sensitivity and specificity were calculated. For SWBCC, the optimal cut-off value with its sensitivity and specificity was calculated with the Youden-Index. Results. Sensitivity and specificity of SFC\(_{(culture)}\) were 0.0% and 98.9%. Sensitivity and specificity of SFC\(_{(histo)}\) were 3.4% and 100%. The optimal cut-off value for SWBCC\(_{(culture)}\) was 4450 cells/μl with a sensitivity of 50.0% and a specificity of 86.5%. The optimal cut-off value for SWBCC\(_{(histo)}\) was 3250 cells/μl with a sensitivity of 35.7% and a specificity of 92.9%. Conclusion. The detection of infection persistence remains challenging and a consented approach is lacking. The results do not warrant the routine performance of SFC during the two-stage exchange at the knee. SWBCC can be used to confirm infection persistence at high cut-offs, but they only occur in few patients and are therefore inappropriate for the routine use.
Die Osteochondrosis dissecans ist eine aseptische Erkrankung des subchondralen Knochens, welche am häufigsten an konvex geformten Gelenkanteilen auftritt und deren Prognose vorwiegend vom Stadium und vom Patientenalter abhängt. Anhand von 80 Patientendaten wurden die Ergebnisse nach stadienabhängiger Therapie der Osteochondrosis dissecans des Kniegelenkes nach einem durchschnittlichen Zeitraum von 7,5 Jahren untersucht. Die Geschlechterverteilung lag bei 1:5 zugunsten der männlichen Personen, das Durchschnittsalter betrug 26,9 Jahr. Die Ergebnisse wurden als Patientenbefindlichkeit über verschiedene Fragebögen und eine klinische Untersuchung ermittelt.
Ziel dieser Arbeit ist, die Wertigkeit von Entzündungsparametern (CRP, BSG, Leukozyten und Temperatur) in der postoperativen Phase anhand einer retrospektiven Studie mit 531 Patienten nach Implantation von Knie- und Hüft-Totalendoprothesen zu beurteilen. Hierbei sollten zum einen die Auswirkungen von Vorerkrankungen (Asthma bronchiale, Rheumatoider Arthritis, Nikotinabusus, Gicht/Hyperurikämie, Z. n. Thrombosen) auf den postoperativen Verlauf der Entzündungsparameter und zum anderen postoperative Komplikationen (Thrombosen, bronchopulmonale Infekte, Harnwegsinfekte, Wundinfekte, Protheseninfekte) anhand der Entzündungsparameter erfasst werden. Die Auswertung erfolgte mittels SPSS für Windows durch den Mann-Whitney Test als nicht parametrischen Test für unabhängige Variablen. Im Durchschnitt stieg der CRP-Wert von 0,6 mg/dl präoperativ auf etwa 8,5 mg/dl um den 3. postoperativen Tag an und fiel dann durchschnittlich auf 1,4 mg/dl um den 14. postoperativen Tag ab. Die BSG-Werte stiegen von etwa 18 mm/h präoperativ auf 52 mm/h um den 3. postoperativen Tag an. Der höchste BSG-Wert (56 mm/h) wurde erst um den 7. postoperativen Tag erreicht. Danach fiel der BSG-Wert leicht ab auf durchschnittlich etwa 43 mm/h um den 14. postoperativen Tag. Die Temperaturkurve verlief ähnlich: Auch hier stieg die Temperatur rasch auf einen Höchstwert von 37,1°C um den 2. postoperativen Tag. Es folgte ein langsamer Abfall der Temperatur, bis hin zu Normwerten ab dem 10. postoperativen Tag. Die Leukozyten zeigten im Durchschnitt einen geradlinigen Verlauf mit durchschnittlichen Werten zwischen 7 und 8 G/l, allerdings bei einer großen Standartabweichung. Patienten mit Asthma bronchiale zeigten präoperativ signifikant erhöhte BSG und Leukozytenwerte. Patienten mit Gicht/Hyperurikämie fielen durch signifikant niedrigere Temperaturwerte am 6. und 12. postoperativen Tag sowie durch Trends zu niedrigeren Werten am 10. und 16. postoperativen Tag auf. Signifikant höhere Werte fanden sich bei der CRP-AUC-Kurve vom 7.-14. postoperativen Tag. Des Weiteren fanden sich Trends zu höheren Werten am 14. postoperativen Tag bei der BSG und am 3. postoperativen Tag bei den Leukozyten. Bei Patienten mit Z.n. Thrombose zeigte sich der postoperative BSG-Verlauf vom 3.-14. Tag signifikant erniedrigt, ebenso waren die postoperativen Temperaturwerte vom 15. und 16. Tag signifikant niedriger. Der einzig signifikante Unterschied zwischen Rauchern und Nichtrauchern fand sich in der präoperativen BSG, wo Raucher signifikant niedrigere Werte hatten. Des Weiteren zeigten sich Trends zu höheren Leukozytenwerten am 3. und 7. postoperativen Tag sowie Trends zu niedrigeren Temperaturen am 1., 2., 7., 8. und 14. postoperativen Tag. Patienten mit Rheumatoider Arthritis fielen durch signifikant erhöhte präoperative CRP- und BSG-Werte auf. Zusätzlich traten Trends zu höheren Werten am 14. postoperativen Tag für CRP, BSG und Leukozyten auf. Dem Trend zur Temperaturerhöhung am 2. postoperativen Tag folgten erneute Trends am 9. und 11. postoperativen Tag sowie signifikant höhere Temperaturen am 10. und 12. postoperativen Tag. Patienten, die einen Harnwegsinfekt erlitten, hatten signifikant erhöhte Werte am 14. postoperativen Tag für CRP und BSG. Auch die CRP-AUC-Kurve zeigte diesen Verlauf signifikant. Bei Patienten, die an einem bronchopulmonalem Infekt erkrankten, war der CRP-Wert des 3. postoperativen Tages signifikant erhöht. Dies ließ sich auch in der CRP-AUC-Kurve vom 0.-3. postoperativen Tag signifikant nachweisen. Darüber hinaus lag ein Trend am 3.-7. postoperativen Tag zu höheren Werten vor. Bei Patienten mit tiefer Beinvenenthrombose fiel eine signifikante Temperaturerhöhung am 7., 8. und 11. postoperativen Tag auf, sowie ein Trend am 9., 10. und 12. postoperativen Tag. Die CRP-Werte stiegen um den 14. postoperativen Tag signifikant an. Gleiches Verhalten zeigte die CRP-AUC-Kurve vom 7.-14. postoperativen Tag. Patienten mit Wundinfektionen fielen durch Trends zu niedrigeren Temperaturwerten am 4. und 5. postoperativen Tag auf. Außerdem stieg um den 7. postoperativen Tag die BSG trendwertig an. Die Leukozyten waren präoperativ signifikant erhöht. Dagegen war bei Patienten mit TEP-Infektion der CRP-Wert um den 14. postoperativen Tag signifikant erhöht. Die Temperaturwerte des 8. postoperativen Tages zeigten einen Trend zu niedrigeren Zahlen. Die Ergebnisse entsprechen in etwa denen anderer Studien. Insgesamt lässt sich eine Tendenz ablesen: BSG und Temperatur sind vor allem bei den erwähnten Vorerkrankungen erhöht, Leukozyten spielen kaum eine Rolle und die CRP-Bestimmung findet ihren Platz hauptsächlich in der Diagnostik von Komplikationen. Außerdem wird klar, dass kein Entzündungsparameter spezifisch für eine bestimmte Vorerkrankung oder Komplikation ist. Darüber hinaus sind nicht Einzelwerte, sondern viel mehr der Verlauf der Entzündungsparameter entscheidend.
Die Kniegelenksinnervation von 6 Mäusen wurde elektronenmikroskopisch untersucht. Der Mediale Artikuläre Nerv (MAN) enthielt durchschnittlich 75 Nervenfasern, 63 unmyelinisierte und 12 myelinisierte (Durchmesser 1 bis 8 µm, Maximum zwischen 2 und 5 µm). Der Posteriore Artikuläre Nerv (PAN) bestand durchschnittlich aus 195 Nervenfasern, 129 unmyelinisierte und 66 myelinisierte (Durchmesser zwischen 1 und 12 µm, Maximum bei 4 bis 5 µm). Diese Daten sprechen für eine weitgehende histologische Vergleichbarkeit der Kniegelenksinnervation bei Maus, Ratte und Katze. Lediglich die Anzahl der Nervenfasern ist bei der im Verhältnis kleineren Maus geringer. Spinalganglienzellen von 10 Mäusen wurden mittels hypoosmolarer Lösung gedehnt. Schwankungen der intrazellulären Kalzium-Konzentrationen und elektrophysiologische Veränderungen wurden dabei registriert (Calcium-Imaging und Patch-Clamp). Die primär sensorischen Neuronen, die nicht an der Kniegelenksinnervation beteiligt waren, konnten in schnell, langsam und nicht reagierende Subpopulationen unterteilt werden. Die Beobachtungen an den retrograd markierten Kniegelnksafferenzen erlaubten eine solche Differenzierung nicht. Die Reizung mit Capsaicin zeigte, dass es sich bei den mechanosensitiven Kniegelenksafferenzen selten um polymodale Nozizeptoren handelte.
