Ziel dieser Untersuchung war, den Einfluss von Interleukin (IL)-1 beta auf die Entwicklung menschlicher Präimplantationsembryonen bei der künstlichen Befruchtung zu überprüfen. An der Studie nahmen 256 Paare teil, die aufgrund verschiedener Indikationen das IVF-/ICSI-Programm der Universitäts-Frauenklinik Würzburg durchliefen. In 160 Fällen fand eine konventionelle IVF-Behandlung statt und in 96 Fällen wurde die ICSI-Methode angewendet. In der Follikelflüssigkeit wurden sowohl die IL-1 beta-Konzentrationen, als auch die FSH-, LH-, Östradiol- und Progesteron-Werte bestimmt. Der Embryotransfer fand nach einer Kulturdauer von fünf Tagen statt. Die Qualität der Embryonen wurde sowohl 14 bis 18 Stunden nach Insemination als auch am Tag 3 und Tag 5 der Embryonenkultur beurteilt. Sofern ausreichend Material vorhanden war, wurden die IL-1 beta-Konzentrationen auch in den Überständen der Embryo-Kulturmedien von Tag 3 und Tag 5 bestimmt. In IVF-Zyklen konnte zwischen IL-1 beta in der Follikelflüssigkeit und der Fertilisationsrate ein signifikanter Zusammenhang festgestellt werden. Befruchteten sich über 90 % der Eizellen, so lagen die zugehörigen IL-1 beta-Konzentrationen signifikant höher (3,34 pg/ml) als bei niedrigeren Befruchtungsraten < 90 % (1,96 pg/ml, p = 0,032*). Bei der Intrazytoplasmatischen Spermieninjektion konnte diese Assoziation nicht nachgewiesen werden. IL-1 beta in der Follikelflüssigkeit scheint mit der Embryonenqualität nicht in Verbindung zu stehen. Weder mit der Zygotenqualität noch mit der morphologischen Entwicklung der Embryonen am dritten oder fünften Kulturtag zeigte sich eine signifikante Korrelation. Auch zwischen dem Eintritt einer Schwangerschaft und den IL-1 beta-Werten in der Follikelflüssigkeit bestand keine signifikante Abhängigkeit. Zwischen IL-1 beta und Hormonen in der Follikelflüssigkeit konnte kein Zusammenhang ermittelt werden. Die IL-1 beta-Werte im Kulturmedium von Tag 3 zeigten keine Korrelation mit der zugehörigen Embryonenmorphologie. Im Tag 5-Medium von Blastozysten mit einer guten morphologischen Qualität und einer mittleren Blastozystenbeurteilung von 3,5 oder besser lagen die IL-1 beta-Werte signifikant höher (29,3 pg/ml) als beim Restkollektiv (4,4 pg/ml, p = 0,041*). In dieser Studie waren hohe intrafollikuläre IL-1 beta-Konzentrationen bei IVF-Patientinnen mit einer erfolgreichen Befruchtung assoziiert. Die Bestimmung dieses Zytokins in der Follikelflüssigkeit scheint allerdings keinen Vorhersagewert für die weitere morphologische Entwicklung der Embryonen oder den Erfolg der IVF/ICSI-Behandlung zu haben.
Acute ischemic cardiac injury predisposes one to cognitive impairment, dementia, and depression. Pathophysiologically, recent positron emission tomography data suggest astroglial activation after experimental myocardial infarction (MI). We analyzed peripheral surrogate markers of glial (and neuronal) damage serially within 12 months after the first ST-elevation MI (STEMI). Serum levels of glial fibrillary acidic protein (GFAP) and neurofilament light chain (NfL) were quantified using ultra-sensitive molecular immunoassays. Sufficient biomaterial was available from 45 STEMI patients (aged 28 to 78 years, median 56 years, 11% female). The median (quartiles) of GFAP was 63.8 (47.0, 89.9) pg/mL and of NfL 10.6 (7.2, 14.8) pg/mL at study entry 0–4 days after STEMI. GFAP after STEMI increased in the first 3 months, with a median change of +7.8 (0.4, 19.4) pg/mL (p = 0.007). It remained elevated without further relevant increases after 6 months (+11.7 (0.6, 23.5) pg/mL; p = 0.015), and 12 months (+10.3 (1.5, 22.7) pg/mL; p = 0.010) compared to the baseline. Larger relative infarction size was associated with a higher increase in GFAP (ρ = 0.41; p = 0.009). In contrast, NfL remained unaltered in the course of one year. Our findings support the idea of central nervous system involvement after MI, with GFAP as a potential peripheral biomarker of chronic glial damage as one pathophysiologic pathway.
Ultra-high field cardiac MRI in large animals and humans for translational cardiovascular research

A key step in translational cardiovascular research is the use of large animal models to better understand normal and abnormal physiology, to test drugs or interventions, or to perform studies which would be considered unethical in human subjects. Ultrahigh field magnetic resonance imaging (UHF-MRI) at 7 T field strength is becoming increasingly available for imaging of the heart and, when compared to clinically established field strengths, promises better image quality and image information content, more precise functional analysis, potentially new image contrasts, and as all in-vivo imaging techniques, a reduction of the number of animals per study because of the possibility to scan every animal repeatedly. We present here a solution to the dual use problem of whole-body UHF-MRI systems, which are typically installed in clinical environments, to both UHF-MRI in large animals and humans. Moreover, we provide evidence that in such a research infrastructure UHF-MRI, and ideally combined with a standard small-bore UHF-MRI system, can contribute to a variety of spatial scales in translational cardiovascular research: from cardiac organoids, Zebra fish and rodent hearts to large animal models such as pigs and humans. We present pilot data from serial CINE, late gadolinium enhancement, and susceptibility weighted UHF-MRI in a myocardial infarction model over eight weeks. In 14 pigs which were delivered from a breeding facility in a national SARS-CoV-2 hotspot, we found no infection in the incoming pigs. Human scanning using CINE and phase contrast flow measurements provided good image quality of the left and right ventricle. Agreement of functional analysis between CINE and phase contrast MRI was excellent. MRI in arrested hearts or excised vascular tissue for MRI-based histologic imaging, structural imaging of myofiber and vascular smooth muscle cell architecture using high-resolution diffusion tensor imaging, and UHF-MRI for monitoring free radicals as a surrogate for MRI of reactive oxygen species in studies of oxidative stress are demonstrated. We conclude that UHF-MRI has the potential to become an important precision imaging modality in translational cardiovascular research.
