Alpha2-Rezeptoren, die weiter in alpha2A, alpha2B und alpha2C unterteilt werden, gehören zur Gruppe der adrenergen Rezeptoren innerhalb der Klasse der G-Protein-gekoppelten Rezeptoren. Sie sind maßgeblich an der Regulation vieler physiologischer Prozesse beteiligt. Vieles, was heute über alpha2-Rezeptoren bekannt ist, wurde mithilfe von alpha2-defizienten Mäusen, sogenannten „Knock-Out“-Mäusen (KO) herausgefunden, von denen bislang drei Einzel-KOs und der Doppel-KO der Subtypen A und C existieren. Im Rahmen dieser Arbeit wurden durch Kreuzung der vorhandenen KO-Linien Mauslinien generiert, die defizient für alpha2A und alpha2B, für alpha2B und alpha2C oder alle drei alpha2-Rezeptoren sind. Während alpha2AB-KO-Mäuse ungefähr entsprechend der Mendelschen Verteilung geboren wurden, zeigte sich, dass alpha2BC-KO-Mäuse teilweise und alpha2ABC-KO-Mäuse sogar komplett embryonal letal waren. Die morphologischen Unter-suchungen legten den Zeitpunkt der embryonalen Letalität der alpha2ABC-KO-Mäuse auf den Tag E10,5 der Embryonalentwicklung fest und konnten zeigen, dass diese Letalität in einem Vaskularisierungsdefekt innerhalb der extraembryonalen Organe Plazenta und Dottersack begründet lag. Diese Organe stellen die Versorgung des Embryos mit Nährstoffen und Sauerstoff sicher und sorgen somit für dessen Entwicklung. Durch RT-PCR-Experimente konnte die mRNS für alle drei alpha2-Rezeptorsubtypen an Tag E10,5 sowohl im Embryo als auch in Plazenta und Dottersack nachgewiesen werden. Autoradiographische Experimente und Radioligandenbindungsstudien an Plazenten machten deutlich, dass der Großteil an alpha2-Rezeptoren im embryonalen Teil der Plazenta exprimiert wird, nämlich in den Riesenzellen und in der sich daran anschließenden Spongiotrophoblastschicht, und dass hierbei alpha2-Rezeptoren vom B-Subtyp vorherrschen. In den genannten Zellen konnte mittels Immunhistochemie eine alpha2-Rezeptor-vermittelte Phosphorylierung der MAP-Kinasen ERK1/2 gezeigt werden, die auch in kultivierten WT-Dottersäcken beobachtet werden konnte. Unter basalen Bedingungen zeigte sich, dass die ERK1/2-Phosphorylierung in Gewebe von alpha2ABC-KO-Embryonen drastisch vermindert war, während andere Signalwege, die von alpha2-Rezeptoren angestoßen werden können, nicht beeinträchtigt waren. Versuche in einem Zellkulturmodell und mit kultivierten WT-Dottersäcken ergaben eine physiologisch relevante Wechselwirkung zwischen dem alpha2B-Rezeptor und dem PDGFbeta-Rezeptor, einer Rezeptortyrosinkinase, als deren Mechanismus sich in Co-Kultur-Experimenten mit alpha2B-Rezeptor-transfizierten Zellen und alpha2ABC-defizienten Dottersäcken die Transaktivierung von Rezeptortyrosinkinasen herausstellte. In dieser Arbeit konnte demonstriert werden, dass a2-Rezeptoren bei der Maus über eine Transaktivierung von ERK1/2 die Vaskularisierung der Plazenta und des Dottersacks bedingen und damit eine normale Embryonalentwicklung sicherstellen.
Objectives
Magnesium phosphate-based cements begin to catch more attention as bone substitute materials and especially as alternatives for the more commonly used calcium phosphates. In bone substitutes for augmentation purposes, atraumatic materials with good biocompatibility and resorbability are favorable. In the current study, we describe the in vivo testing of novel bone augmentation materials in form of spherical granules based on a calcium-doped magnesium phosphate (CaMgP) cement.
Materials and Methods
Granules with diameters between 500 and 710 μm were fabricated via the emulsification of CaMgP cement pastes in a lipophilic liquid. As basic material, two different CaMgP formulations were used. The obtained granules were implanted into drill hole defects at the distal femoral condyle of 27 New Zealand white rabbits for 6 and 12 weeks. After explantation, the femora were examined via X-ray diffraction analysis, histological staining, radiological examination, and EDX measurement.
Results
Both granule types display excellent biocompatibility without any signs of inflammation and allow for proper bone healing without the interposition of connective tissue. CaMgP granules show a fast and continuous degradation and enable fully adequate bone regeneration.
Conclusions
Due to their biocompatibility, their degradation behavior, and their completely spherical morphology, these CaMgP granules present a promising bone substitute material for bone augmentation procedures, especially in sensitive areas.
Clinical Relevance
The mostly insufficient local bone supply after tooth extractions complicates prosthetic dental restoration or makes it even impossible. Therefore, bone augmentation procedures are oftentimes inevitable. Spherical CaMgP granules may represent a valuable bone replacement material in many situations.
Usability of a mHealth solution using speech recognition for point-of-care diagnostic management

The administrative burden for physicians in the hospital can affect the quality of patient care. The Service Center Medical Informatics (SMI) of the University Hospital Würzburg developed and implemented the smartphone-based mobile application (MA) ukw.mobile1 that uses speech recognition for the point-of-care ordering of radiological examinations. The aim of this study was to examine the usability of the MA workflow for the point-of-care ordering of radiological examinations. All physicians at the Department of Trauma and Plastic Surgery at the University Hospital Würzburg, Germany, were asked to participate in a survey including the short version of the User Experience Questionnaire (UEQ-S) and the Unified Theory of Acceptance and Use of Technology (UTAUT). For the analysis of the different domains of user experience (overall attractiveness, pragmatic quality and hedonic quality), we used a two-sided dependent sample t-test. For the determinants of the acceptance model, we employed regression analysis. Twenty-one of 30 physicians (mean age 34 ± 8 years, 62% male) completed the questionnaire. Compared to the conventional desktop application (DA) workflow, the new MA workflow showed superior overall attractiveness (mean difference 2.15 ± 1.33), pragmatic quality (mean difference 1.90 ± 1.16), and hedonic quality (mean difference 2.41 ± 1.62; all p < .001). The user acceptance measured by the UTAUT (mean 4.49 ± 0.41; min. 1, max. 5) was also high. Performance expectancy (beta = 0.57, p = .02) and effort expectancy (beta = 0.36, p = .04) were identified as predictors of acceptance, the full predictive model explained 65.4% of its variance. Point-of-care mHealth solutions using innovative technology such as speech-recognition seem to address the users’ needs and to offer higher usability in comparison to conventional technology. Implementation of user-centered mHealth innovations might therefore help to facilitate physicians’ daily work.
