Purpose: In hepatocellular carcinoma patients with large or multinodal tumors, where curative treatment options are not feasible, transarterial therapies play a major role. Transarterial chemoembolization (TACE) with drug-eluting beads (DEB-TACE) is a promising new approach due to higher intratumoral and lower systemic concentration of the chemotherapeutic agent compared to conventional TACE (cTACE).
Patients and methods: In a retrospective analysis, 32 patients with hepatocellular carcinoma who received either DEB or a cTACE were compared regarding survival time, disease recurrence, and side effects such as pain and fever.
Results: No significant differences could be detected between the cTACE and DEB-TACE groups with regard to mean hospital stay, appearance of postinterventional fever, or 30-day mortality. However, the application of intravenous analgesics as postinterventional pain medication was needed more often in patients treated with DEB-TACE (57.1% vs 12.5%, P=0.0281). The overall median survival after the initial procedure was 10.8 months in the cTACE group and 9.2 months in the DEB-TACE group, showing no significant difference.
Conclusion: No survival benefit for patients treated with either DEB-TACE or cTACE was observed. Surprisingly, a higher rate of postinterventional pain could be detected after DEB-TACE.
Interferon alpha (IFNα) is approved for adjuvant treatment of stage III melanoma in Europe and the US. Its clinical efficacy, however, is restricted to a subpopulation of patients while side effects occur in most of treated patients. Thus, the identification of predictive biomarkers would be highly beneficial to improve the benefit to risk ratio. In this regard, STAT3 is important for signaling of the IFNα receptor. Moreover, the STAT3 single-nucleotide polymorphism (SNP) rs4796793 has recently been reported to be associated with IFNα sensitivity in metastatic renal cell carcinoma. To translate this notion to melanoma, we scrutinized the impact of rs4796793 functionally and clinically in this cancer. Interestingly, melanoma cells carrying the minor allele of rs4796793 were the most sensitive to IFNα in vitro. However, we did not detect a correlation between SNP genotype and STAT3 mRNA expression for either melanoma cells or for peripheral blood lymphocytes. Next, we analyzed the impact of rs4796793 on the clinical outcome of 259 stage III melanoma patients of which one-third had received adjuvant IFNα treatment. These analyses did not reveal a significant association between the STAT3 rs4796793 SNP and patients' progression free or overall survival when IFNα treated and untreated patients were compared. In conclusion, STAT3 rs4796793 SNP is no predictive marker for the efficacy of adjuvant IFNα treatment in melanoma patients.
Merkel cell carcinoma (MCC) is a highly aggressive skin cancer with an increasing incidence. The understanding of the molecular carcinogenesis of MCC is limited. Here, we scrutinized the PI3K/AKT pathway, one of the major pathways activated in human cancer, in MCC. Immunohistochemical analysis of 41 tumor tissues and 9 MCC cell lines revealed high levels of AKT phosphorylation at threonine 308 in 88% of samples. Notably, the AKT phosphorylation was not correlated with the presence or absence of the Merkel cell polyoma virus (MCV). Accordingly, knock-down of the large and small T antigen by shRNA in MCV positive MCC cells did not affect phosphorylation of AKT. We also analyzed 46 MCC samples for activating PIK3CA and AKT1 mutations. Oncogenic PIK3CA mutations were found in 2/46 (4%) MCCs whereas mutations in exon 4 of AKT1 were absent. MCC cell lines demonstrated a high sensitivity towards the PI3K inhibitor LY-294002. This finding together with our observation that the PI3K/AKT pathway is activated in the majority of human MCCs identifies PI3K/AKT as a potential new therapeutic target for MCC patients.
Pathomechanismen von Antikörpern gegen Aquaporin 4 in einem Tiermodell für die Neuromyelitis Optica

Die Neuromyelitis Optica (NMO) ist eine schwerwiegende autoimmune Erkrankung des zentralen Nervensystems (ZNS), die mit rezidivierenden Optikusneuritiden und Querschnittsmyelitiden einhergeht. Als serologischer Biomarker wurden Autoantikörper gegen Aquaporin 4 (anti-AQP4-AK) identifiziert. Mit Hilfe eines passiv-Transfer Rattenmodelles mit implantierten intrathekalen Kathetern wurden aufgereinigte IgG Fraktionen (NMO-IgG) von Plasmapheresematerial anti-AQP4-AK positiver NMO Patienten verabreicht. Zum Nachweis der Antigen-Spezifität wurden in weiteren Versuchsgruppen rekombinante IgG-AK gegen AQP4 appliziert. Die repetitive Injektion von NMO-IgG oder anti-AQP4-AK führte zu einer signifikanten klinischen Verschlechterung und einer reduzierten motorischen Leistungsfähigkeit der Versuchstiere im Vergleich zu Kontrollen. Mittels Magnetresonanztomographie konnten exemplarisch Kontrastmittel-aufnehmende Läsionsareale im Rückenmark der Versuchstiere im Bereich der Katheterspitze detektiert werden. Histopathologisch zeigte sich in diesen Läsionsbereichen eine Anreicherung von intrathekal applizierten humanen IgG, ein Verlust der Expression von AQP4 und des Glutamattransporters EAAT2. Im Gegensatz zu der bisher bekannten, Komplement-induzierten Gewebedestruktion bei NMO-Patienten mit entzündlichen Läsionen wurde hier keine Depletion von Astrozyten oder Komplementaktivierung beobachtet. Stattdessen kam es in den hier beschriebenen Arealen mit IgG-Ablagerung zu einer Hypertrophie und Vermehrung der GFAP-positiven Astrozyten. Die Ergebnisse lassen auf eine pathophysiologisch relevante, intrinsische und komplement-unabhängige Wirkung von anti-AQP4-AK schließen.
Diese Arbeit zeigt den zeitlichen Verlauf der geometrischen und funktionellen kardialen Änderungen während chronischer Druckinduktion des Mäuseherzens nach kontrollierter Banding-Operation der Aorta thoracalis im Langzeitverlauf von insgesamt 26 Wochen. Als Kontrollgruppe dienten Tiere, die sich zum gleichen Zeitpunkt einer Pseudo-Operation (Sham-Operation) unterzogen. Zur Erfassung der kardialen Funktionsparameter sowie der Dynamik wurde schnelle hochauflösende MR-Kinobildgebung in einem 7 Tesla Magneten mit Hilfe eines Mikroskopie-Gradientensystems unter Isofluran-Narkose durchgeführt. Erfasst wurden hierbei die Parameter enddiastolisches Volumen (EDV), endsytolisches Volumen (ESV), Schlagvolumen (SV), Auswurffraktion (EF), Herzminutenvolumen (HMV), Herzindex (CI), linksventrikuläre (LV) Masse, LV-Massenindex, LV-Wanddicke endsytolisch und enddiastolisch sowie LV-Entleerungs- und Füllungsrate. Zusätzliche Zeit-Volumen-Kurven wurden berechnet. Zu jedem Untersuchungszeitpunkt wurde eine histologische Aufarbeitung des linken Ventrikels durchgeführt. Die Kardiomyozytengröße wurde nach einer Hämalaun-Eosin-(HE)-Färbung planimetrisch lichtmikroskopisch bestimmmt. Der Grad der einsetzenden Fibrose wurde mit einer Picro-Sirius-Rot(PSR)-Färbung und nachfolgender Mikroskopie mit zirkulär polarisiertem Licht bestimmt. Es zeigten sich die typischen linksventrikulären Veränderungen der druckinduzierten Hypertrophie, die sich in vier Stufen einteilen lässt: Akutphase bis sieben Tage nach Operation, Kompensationsphase bis zwei Wochen nach Operation, Umbauphase bis sieben Wochen nach Operation und die Dekompensationsphase ab sieben Wochen nach Operation. Die Kollagendichte hatte sich zum Beispiel 26 Wochen nach Operation verdoppelt im Vergleich zur Erstuntersuchung. Die zeitliche, funktionelle und histologische Erfassung der linksventrikulären Hypertrophie ist eine wichtige Grundlage für die Planung weiterer Studien, besonders in Hinblick auf den Einsatz von Modellen mit transgenen Tieren sowie Tieren mit Gen-Knockout. Vorteil der kardialen mikroskopischen Magnetresonanztomographie (MRT) ist der Einsatz als Kontrollmethode der Auswirkungen der genetischen Manipulation nahezu direkt nach der Geburt. Entscheidend ist die umfassende kardiovaskuläre Phänotyp-Charakterisierung der Maus als Schlüssel für die Anwendung der experimentell gewonnen Erkenntnisse zum weiteren Verständnis der Morphologie und funktionellen Abläufe des Herzen sowie der sich daraus ableitenden Therapiemöglichkeiten am menschlichen Herzen. Eine Anwendung dieser Bildgebungsmethode an der Maus ist in der Zukunft zum Beispiel auch denkbar für die Untersuchung von Remodeling-Prozessen nach Myokard-Infarkt, regionaler Herzmuskelfunktion unter Verwendung der Stress-Cinematographie, MR-Tagging- sowie Phasenkontrast-Bildgebung und der myokardialen First-Pass-Perfusionsbildgebung in Kombination mit dem späten Kontrastmittel-Enhancement nach Myokardinfarkt.
Male breast cancer (mBC) is associated with a high prevalence of pathogenic variants (PVs) in the BRCA2 gene; however, data regarding other BC predisposition genes are limited. In this retrospective multicenter study, we investigated the prevalence of PVs in BRCA1/2 and 23 non-BRCA1/2 genes using a sample of 614 patients with mBC, recruited through the centers of the German Consortium for Hereditary Breast and Ovarian Cancer. A high proportion of patients with mBC carried PVs in BRCA2 (23.0%, 142/614) and BRCA1 (4.6%, 28/614). The prevalence of BRCA1/2 PVs was 11.0% in patients with mBC without a family history of breast and/or ovarian cancer. Patients with BRCA1/2 PVs did not show an earlier disease onset than those without. The predominant clinical presentation of tumor phenotypes was estrogen receptor (ER)-positive, progesterone receptor (PR)-positive, and HER2-negative (77.7%); further, 10.2% of the tumors were triple-positive, and 1.2% were triple-negative. No association was found between ER/PR/HER2 status and BRCA1/2 PV occurrence. Comparing the prevalence of protein-truncating variants (PTVs) between patients with mBC and control data (ExAC, n = 27,173) revealed significant associations of PTVs in both BRCA1 and BRCA2 with mBC (BRCA1: OR = 17.04, 95% CI = 10.54–26.82, p < 10\(^{−5}\); BRCA2: OR = 77.71, 95% CI = 58.71–102.33, p < 10\(^{−5}\)). A case-control investigation of 23 non-BRCA1/2 genes in 340 BRCA1/2-negative patients and ExAC controls revealed significant associations of PTVs in CHEK2, PALB2, and ATM with mBC (CHEK2: OR = 3.78, 95% CI = 1.59–7.71, p = 0.002; PALB2: OR = 14.77, 95% CI = 5.02–36.02, p < 10\(^{−5}\); ATM: OR = 3.36, 95% CI = 0.89–8.96, p = 0.04). Overall, our findings support the benefit of multi-gene panel testing in patients with mBC irrespective of their family history, age at disease onset, and tumor phenotype.
Background
In spite of several research studies help to describe the heart in Fabry disease (FD), the cardiomyopathy is not entirely understood. In addition, the impact of blood pressure and alterations in geometry have not been systematically evaluated.
Methods
In 74 FD patients (mean age 36±12 years; 45 females) the extent of myocardial fibrosis and its progression were quantified using cardiac magnetic-resonance-imaging with late enhancement technique (LE). Results were compared to standard echocardiography complemented by 2D-speckle-tracking, 3D-sphericity-index (SI) and standardized blood pressure measurement. At baseline, no patient received enzyme replacement therapy (ERT). After 51±24 months, a follow-up examination was performed.
Results
Systolic blood pressure (SBP) was higher in patients with vs. without LE: 123±17 mmHg vs. 115±13 mmHg; P = 0.04. A positive correlation was found between SI and the amount of LE-positive myocardium (r = 0.51; P<0.001) indicating an association of higher SI in more advanced stages of the cardiomyopathy. SI at baseline was positively associated with the increase of LE-positive myocardium during follow-up. The highest SBP (125±19 mmHg) and also the highest SI (0.32±0.05) was found in the subgroup with a rapidly increasing LE (ie, ≥0.2% per year; n = 16; P = 0.04). Multivariate logistic regression analysis including SI, SBP, EF, left ventricular volumes, wall thickness and NT-proBNP adjusted for age and sex showed SI as the most powerful parameter to detect rapid progression of LE (AUC = 0.785; P<0.05).
Conclusions
LV geometry as assessed by the sphericity index is altered in relation to the stage of the Fabry cardiomyopathy. Although patients with FD are not hypertensive, the SBP has a clear impact on the progression of the cardiomyopathy.
Background
The effect of smoking on coronary vasomotion has been investigated in the past with various imaging techniques in both short- and long-term smokers. Additionally, coronary vasomotion has been shown to be normalized in long-term smokers by L-Arginine acting as a substrate for NO synthase, revealing the coronary endothelium as the major site of abnormal vasomotor response. Aim of the prospective cohort study was to investigate coronary vasomotion of young healthy short-term smokers via magnetic resonance cold pressor test with and without the administration of L-Arginine and compare obtained results with the ones from nonsmokers.
Methods
Myocardial blood flow (MBF) was quantified with first-pass perfusion MRI on a 1.5 T scanner in healthy short-term smokers (N = 10, age: 25.0 ± 2.8 years, 5.0 ± 2.9 pack years) and nonsmokers (N = 10, age: 34.3 ± 13.6) both at rest and during cold pressor test (CPT). Smokers underwent an additional examination after administration of L-Arginine within a median of 7 days of the naïve examination.
Results
MBF at rest turned out to be 0.77 ± 0.30 (smokers with no L-Arginine; mean ± standard deviation), 0.66 ± 0.21 (smokers L-Arginine) and 0.84 ± 0.08 (nonsmokers). Values under CPT were 1.21 ± 0.42 (smokers no L-Arginine), 1.09 ± 0.35 (smokers L-Arginine) and 1.63 ± 0.33 (nonsmokers). In all groups, MBF was significantly increased under CPT compared to the corresponding rest examination (p < 0.05 in all cases). Additionally, MBF under CPT was significantly different between the smokers and the nonsmokers (p = 0.002). MBF at rest was significantly different between the smokers when L-Arginine was given and the nonsmokers (p = 0.035).
Conclusion
Short-term smokers showed a reduced response to cold both with and without the administration of L-Arginine. However, absolute MBF values under CPT were lower compared to nonsmokers independently of L-Arginine administration.
