Die Prävalenz von Infektionen mit Neisseria gonorrhoeae (NG) bei HIV-positiven Patientinnen und Patienten einer auf HIV-Infektionen spezialisierten Klinik in Mwanza im Nordwesten Tansani- as erwies sich mit 0,4% bei Männern und 3,9% bei Frauen als relativ niedrig. In dieser aktuellen Studie gab es unter Verwendung molekularer Antibiotikaresistenz(AMR)-Tests keine Hinweise auf eine Resistenz gegen Cephalosporine der dritten Generation. Weitere Kontrollen und Entwicklungen von NG- und AMR-Tests müssen implementiert werden. Molekulare Diagnoseverfahren auf der Basis von Urinproben als diagnostisches Material zum Nachweis von NG weisen wesentliche Vorteile für das Screening auf. Die Durchführung eines Urinstreifentests hatte keinen positiven Vorhersagewert bezüglich einer Infektion mit NG oder einer AMR. Künftige Untersuchungen sind anzuregen, um einerseits eine exaktere Angabe der Prävalenzraten und Risikofaktoren von Infektionen mit NG sowie deren Resistenzlage zu ermöglichen und andererseits eine effizientere Versorgung bzw. Behandlung der Gonorrhoe gewährleisten zu können.
Die Schistosomiasisprävalenz und Krankheitslast in der Region Mwanza ist trotz staatlicher Maßnahmen anhaltend hoch. Zur Vorbereitung einer integralen Eliminationsstudie auf der Insel Ijinga sollte erfasst werden, ob die Bevölkerung über relevante Kenntnisse und Motivation verfügt, um sich an nachhaltiger Schistosomiasiskontrolle zu beteiligen. Zudem sollten lokal priorisierte Maßnahmen, sowie hemmende und unterstützende Faktoren identifiziert werden, um geplante Maßnahmen bestmöglich an die lokalen Begebenheiten anzupassen.
Von Februar bis März 2016 wurde eine quantitative Befragung von 356 Schulkindern, sowie 393 Erwachsenen durchgeführt. Anhand logistischer Regression wurden Faktoren ermittelt die mit einem relevanten Kenntnisstand assoziiert sind. Unter Schulkindern wurden des Weiteren Faktoren bestimmt, die mit einer hohen Motivation an Therapiekampagnen teilzunehmen, einhergehen.
Nur ein geringer Teil der Bevölkerung (17% der Kinder und 48% der Erwachsenen) verfügte über ein Mindestmaß an relevantem Wissen, um sich an nachhaltiger Schistosomiasiskontrolle zu beteiligen. Assoziierte Faktoren waren bei den Kindern die Informationsquelle Schule, der Besuch der 6. und 7. Klassen und eine zurückliegende Diagnose. Bei Erwachsenen bestand ein Zusammenhang zu verschiedenen Informationsquellen (Schule, Gesundheitswesen, lokale Medien), zum Alter, Bildungsstand und zur Präsenz von Symptomen. Nur 40% der Kinder hatten eine hohe Motivation an Therapiekampagnen teilzunehmen. Assoziierte Faktoren waren die wahrgenommene eigene Vulnerabilität und Schwere der Erkrankung, eine zurückliegende Diagnose, sowie eine erwartete Heilung durch Praziquantel.
Die aktuelle Wissensvermittlung in der 6. und 7. Schulklasse erfolgt zu spät und zu punktuell, um über einen theoretischen Kenntnisstand hinaus zur Verhaltensänderung zu führen. Eine systematische Aufnahme von Schistosomiasis in verschiedene Sektoren könnte das Bewusstsein in der Bevölkerung stärken. Zur nachhaltigen Einbindung der Bevölkerung ist ein partizipativer Ansatz, priorisierte WASH Interventionen und eine Stärkung des dezentralen Gesundheitssystems zur frühen Fallfindung und Therapie sinnvoll.
The present study investigates the infection rates of parasites, morbidity, and the living conditions of street children and orphans in Mwanza city, northern Tanzania. A high percentage of orphans and street children in Mwanza city is infected with one or more parasites. A significantly higher rate of infections with S. mansoni in street children as compared with orphans could be observed. The prevalence of S. mansoni determined by POC CCA test was 65.9% for orphans and 94.5% for street children. 19.2% of the orphans tested positive for S. mansoni in Kato Katz. Of the street children, 77.1% showed positive test results in Kato-Katz. Only 1.3% of the orphans stated in the questionnaire that they use the lake to wash, whereas 91.1% of the street children named the lake as at least one of their options for washing. Protozoal infections used as a marker for hygiene were at a comparable level for both groups. Microscopy showed positive results for G. intestinalis in 8.2% and for E. histolytica/dispar in 23% of orphans and 8.1% for G. intestinalis, and 23.8% for E. histolytica/dispar in street children. Through ultrasonography, we observed no signs of severe PPF and only a few mild PPF patterns. Most street children use the lake to wash and often do not have access to adequate sanitation. However, everyone in the study group indicated having access to safe drinking water. Overall, we found the general hygienic conditions for both groups to be inadequate. With the help of simple public health measures, like improve sanitation and regular mass drug administration, the overall situation would likely be considerably improved.
Echinococcosis is an important zoonosis. The causative agent of Alveolar Echinococcosis (AE) is Echinococcus multilocularis. The treatment of human AE is limited to surgery and chemotherapy with albendazole (ABZ). However, ABZ works only parasitostatically and it needs to be taken for long periods, although it causes adverse side effects. Thus, development of new, parasiticidal drug with selective toxicity is required. Because undifferentiated stem cells of E. multilocularis play key role in its longevity and regenerative capacity, targeting stem cells is especially important.
In vitro screening of protein kinases inhibitors demonstrated that human PIM kinases inhibitors have detrimental effects on E. multilocularis. Through yeast two hybrid assay, the interaction of parasite PIM kinase (EmPIM) and its CDC25 (EmCDC25) was indicated. Through in situ hybridization, expression of EmPIM in the stem cells was observed. Therefore, EmPim is likely to be a positive regulator of cell cycle progression, the same as human Pim1. In addition, 20 compounds against EmPIM were selected through in silico screening and synthesized. One of them has a detrimental effect on E.multilocularis comparable to human pan-PIM inhibitors, but has much weaker toxicity on human cell lines.
Furthermore, triclabendazole (TCBZ) and its metabolite TCBZSX, which are approved for another flatworm disease, Fascioliasis were tried on E. multilocularis. With two stem cell markers, damage to stem cells by TCBZSX was shown. In addition, primary cells from treated vesicles never regenerated and the damage to stem cells proved to be irreversible.
Our in silico screening method used in EmPIM research has potential to identify compounds which overcome the side effect problem in ABZ-based chemotherapy. On the other hand, it is expected that my research of TCBZ can lead to development of a practical parasiticidal chemotherapy by combining TCBZ, which damages stem cells, and ABZ, which damages differentiated cells.
Die COVID-19 Pandemie ist die bisher verheerendste Pandemie des 21. Jahrhunderts. Durch die Einführung neuer mRNA-basierter Impfstoffe sowie der hohen Rate natürlicher Infektionen konnte die weltweite SARS-CoV-2-Immunität gesteigert werden. Trotz aller Erfolge zur Eindämmung der Pandemie kann eine Infektion auch heute noch zu schweren Verläufen und Tod führen. Eine adäquate COVID-19-Therapie ist folglich auf potente Virostatika angewiesen. Eine durch Umgehung zeitaufwändiger klinischer Studien schnell verfügbare Alternative zu neu entwickelten Arzneimitteln ist die Anwendung etablierter Medikamente. Wir isolierten und charakterisierten ein von einem Patienten stammendes SARS-CoV-2-Virus. Dieses Virusisolat wurde bisher in elf Publikationen verwendet. Mittels quantitativer Echtzeit-Polymerasekettenreaktion untersuchten wir eine Substanzbibliothek mit mehr als 300 neuen und bereits zugelassenen Wirkstoffen auf ihre Wirksamkeit gegen SARS-CoV-2. Dabei konnten wir zeigen, dass der selektive Serotonin-Wiederaufnahmehemmer Fluoxetin die SARS-CoV-2-Replikation ab einer Dosis von 0,8 μg/ml signifikant inhibiert, einer bei der Behandlung von Depressionen häufig angewandten Dosierung. Der EC50-Wert lag bei 387 ng/ml. Die Behandlung mit Fluoxetin resultierte in einer reduzierten Zahl an Virusprotein-produzierenden Zellen, was darauf hindeutet, dass es die virale Reinfektion und/oder Proteinexpression inhibiert. Fluoxetin ist ein racemisches Gemisch, wobei das (S)-Enantiomer der potentere Serotonin-Wiederaufnahmehemmer ist. Wir konnten zeigen, dass beide Enantiomere einen vergleichbaren antiviralen Effekt gegen SARS-CoV-2 aufweisen, wodurch das (R)-Enantiomer bei virologischer Indikation gegebenenfalls präferiert werden sollte. Fluoxetin hat keinen Einfluss auf die Replikation des Tollwut-Virus und des Humanen Respiratorischen Synzytial-Virus, was auf eine Virusspezifität hindeutet. Weitere aus der Bibliothek stammende signifikante Inhibitoren der SARS-CoV-2-Replikation sind die am Institut für Organische Chemie Würzburg entwickelten Substanzen AKS 232 und AKS 128. Neben der medikamentösen Therapie ist die akkurate Bestimmung neutralisierender Antikörper gegen SARS-CoV-2 zur Quantifizierung des bestehenden (Re-) Infektionsschutzes sowie zur Planung zukünftiger Impfstrategien von großer Bedeutung. Im Rahmen dieser Arbeit entwickelten wir unter Verwendung der quantitativen Echtzeit-Polymerasekettenreaktion erfolgreich ein zuverlässiges Testverfahren zur Detektion neutralisierender anti-SARS-CoV-2 Antikörper.
Even though the international combat against Neglected Tropical Diseases such as schistosomiasis or soil-transmitted helminthiases depends on reliable therapeutics, anthelminthic pharmacovigilance has been neglected on many national African drug markets. Therefore, quality and composition of 88 different batches of Albendazole, Mebendazole and Praziquantel locally collected from randomly selected facilities in Western Burkina Faso, Southeast Côte d’Ivoire, Southwest Ghana and Northwest Tanzania were analysed.
Visual examination of both packaging and samples was performed according to the WHO ‘Be Aware’ tool. Products were then screened with the GPHF Minilab, consisting of tests of mass uniformity, disintegration times and thin-layer chromatography (TLC). Confirmatory tests were performed according to international pharmacopoeiae, applying assays for dissolution profiles and high-performance liquid chromatography (HPLC).
Despite minor irregularities, appearance of the products did not hint at falsified medicines. However, 19.6 % of the brands collected in Ghana and Tanzania were not officially licensed for sale. Mass uniformity was confirmed in 53 out of 58 brands of tablets. 41 out of 56 products passed disintegration times; 10 out of the 15 failing products did not disintegrate at all.
TLC results did not reveal any falsifications or pronounced dosing errors. HPLC findings confirmed the TLC results despite shifted specification limits: ten of the 83 tested batches contained less than 90 %, none more than 110 % label claim. However, no more than 46.3 % (31 / 67) of the tablet batches assayed passed the respective criteria for dissolution.
In the four study countries, no falsified anthelminthic medicine was encountered. The active pharmaceutical ingredient was not found to either exceed or distinctively fall below specification limits. Galenic characteristics as most critical criteria however, especially dissolution profiles, revealed substantial deficits.
