Hintergrund: Depressionen zählen zu den häufigsten psychischen Erkrankungen. Depressive Symptome umfassen beeinträchtigte kognitive Funktionen, vegetative Beschwerden und ein verändertes emotionales Erleben. Die defizitäre Wahrnehmung interner körperlicher Signale wird sowohl mit der Pathogenese der Depression als auch mit Angststörungen in Verbindung gebracht. Interozeptive Genauigkeit (IAc) beschreibt dabei die Fähigkeit, körperliche Empfindungen wie den eigenen Herzschlag akkurat wahrzunehmen und wird mit einer Herzwahrnehmungsaufgabe erfasst. In bildgebenden Verfahren wie der funktionellen Magnetresonanztomografie (fMRT) war eine niedrigere IAc mit einer verringerten Inselaktivität assoziiert. Während der Ruhezustandsmessung des Gehirns (resting-state fMRT) kann in Abwesenheit einer Aufgabe die intrinsische Aktivität des Gehirns gemessen werden. Dies ermöglicht die Identifizierung von kortikalen Netzwerken. Depressive Patienten weisen eine veränderte funktionelle Konnektivität innerhalb und zwischen einzelnen Netzwerken wie dem Salience Network (SN), welchem die Insel zugerechnet wird, und dem Default Mode Network (DMN) auf. Bisherige Studien, in denen überwiegend jüngere depressive Patienten untersucht wurden, kamen jedoch hinsichtlich der IAc und den kortikalen Netzwerken zu inkonsistenten Ergebnissen. Insbesondere ist unklar, inwieweit sich die IAc nach einem Therapieansprechen verändert, von der Herzratenvariabilität (HRV) moduliert wird und welche Auswirkungen dies auf die funktionelle Konnektivität kortikaler Netzwerke hat.
Ziele: Eine veränderte IAc und HRV wie auch funktionelle Konnektivitätsunterschiede im DMN und SN könnten Biomarker der Depression darstellen. Im Rahmen einer Längsschnittuntersuchung wurde getestet, ob ältere depressive Patienten über eine verringerte IAc, eine geringere HRV und über eine veränderte funktionelle Konnektivität im SN sowie DMN verfügen. Darüber hinaus sollte erforscht werden, in welchem Ausmaß sich Patienten, die auf die Behandlung ansprachen (Responder), von sogenannten Non-Respondern in Bezug auf die IAc, die HRV, das SN und das DMN unterschieden.
Methoden: In Studie 1 (Baseline) wurden 30 größtenteils medizierte, schwer depressive Patienten (> 50 Jahre) und 30 gesunde Kontrollprobanden untersucht. Die IAc wurde in einer Herzwahrnehmungsaufgabe ermittelt und die HRV bestimmt. Zusätzlich wurde eine resting-state fMRT durchgeführt. Eine funktionelle Konnektivitätsanalyse für Saatregionen im SN und DMN wurde mit einem saatbasierten Ansatz (seed-to-voxel) durchgeführt. Für eine Subgruppenanalyse wurde die Patientengruppe in ängstlich-depressive und nicht-ängstlich depressive Patienten unterteilt.
In Studie 2 (sechs Monate Follow-up) wurde die Studienkohorte nochmals untersucht. Es nahmen 21 Personen der Patientengruppe und 28 Probanden der Kontrollgruppe teil. Wiederum wurden die IAc und die HRV bestimmt. Außerdem fand eine resting-state fMRT-Messung statt. Die Patientengruppe wurde unterteilt in depressive Responder und Non-Responder.
Ergebnisse: In Studie 1 zeigten depressive Patienten eine funktionelle Hypokonnektivität zwischen einzelnen Saatregionen der Insel (SN) und Teilen des superioren frontalen Gyrus, des supplementärmotorischen Cortex, des lateralen okzipitalen Cortex sowie des Okzipitalpols. Zudem wiesen depressive Patienten zwischen der Saatregion im anterioren Teil des DMN und der Insel sowie dem Operculum eine erhöhte funktionelle Konnektivität auf. Die Gruppen unterschieden sich nicht in der IAc und der HRV. Ängstlich-depressive Patienten zeigten eine höhere funktionelle Konnektivität innerhalb der Insel als nicht-ängstlich depressive Patienten, jedoch zeigten sich keine Unterschiede in der IAc und der HRV.
In Studie 2 wiesen depressive Non-Responder im Vergleich zu Respondern eine Hyperkonnektivität zwischen dem posterioren DMN und dem Frontalpol sowie zwischen dem posterioren DMN und temporalen Arealen im SN auf. Keine funktionellen Konnektivitätsunterschiede zeigten sich für die Saatregionen im SN. Depressive Responder, Non-Responder und die Kontrollprobanden unterschieden sich in ihrer IAc und HRV nicht.
Schlussfolgerungen: Die Ergebnisse der Studien unterstreichen, dass bei depressiven Patienten, Respondern und Non-Respondern Unterschiede in der intrinsischen Gehirnaktivität funktioneller Netzwerke bestehen, jedoch nicht in der akkuraten Wahrnehmung des eigenen Herzschlages und der HRV. Therapeutische Interventionen, die auf eine Verbesserung der IAc abzielen, könnten insbesondere für Non-Responder dennoch eine zusätzliche Behandlungsmöglichkeit darstellen. Für eine personalisierte Medizin könnte die weitere Erforschung von kortikalen Netzwerken einen wesentlichen Beitrag leisten, um ein individuelles Therapieansprechen zu prädizieren.
Emotional-associative learning processes such as fear conditioning and extinction are highly relevant to not only the development and maintenance of anxiety disorders (ADs), but also to their treatment. Extinction, as the laboratory analogue to behavioral exposure, is assumed a core process underlying the treatment of ADs. Although exposure-based treatments are highly effective for the average patient suffering from an AD, there remains a gap in treatment efficacy with over one third of patients failing to achieve clinically significant symptom relief. There is ergo a pressing need for intensified research regarding the underlying neural mechanisms of aberrant emotional-associative learning processes and the neurobiological moderators of treatment (non-)response in ADs.
The current thesis focuses on different applications of the fundamental principles of fear conditioning and extinction by using two example cases of ADs from two different multicenter trials. First, we targeted alterations in fear acquisition, extinction, and its recall as a function of psychopathology in panic disorder (PD) patients compared to healthy subjects using fMRI. Second, exposure-based therapy and pre-treatment patient characteristics exerting a moderating influence on this essential learning process later on (i.e. treatment outcome) were examined using multimodal functional and structural neuroimaging in spider phobia.
We observed aberrations in emotional-associative learning processes in PD patients compared to healthy subjects indicated by an accelerated fear acquisition and an attenuated extinction recall. Furthermore, pre-treatment differences related to defensive, regulatory, attentional, and perceptual processes may exert a moderating influence on treatment outcome to behavioral exposure in spider phobia. Although the current results need further replication, on an integrative meta level, results point to a hyperactive defensive network system and deficient emotion regulation processes (including extinction processes) and top-down control in ADs. This speaks in favor of transdiagnostic deficits in important functional domains in ADs.
Deficits in transdiagnostic domains such as emotion regulation processes could be targeted by enhancing extinction learning or by means of promising tools like neurofeedback. The detection of pre-treatment clinical response moderators, for instance via machine learning frameworks, may help in supporting clinical decision making on individually tailored treatment approaches or, respectively, to avoid ineffective treatment and its related financial costs. In the long run, the identification of neurobiological markers which are capable of detecting non-responders a priori represents an ultimate goal.
Empathy, the act of sharing another person’s affective state, is a ubiquitous driver for helping others and feeling close to them. These experiences are integral parts of human behavior and society. The studies presented in this dissertation aimed to investigate the sustainability and stability of social closeness and prosocial decision-making driven by empathy and other social motives. In this vein, four studies were conducted in which behavioral and neural indicators of empathy sustainability were identified using model-based functional magnetic resonance imaging (fMRI).
Applying reinforcement learning, drift-diffusion modelling (DDM), and fMRI, the first two studies were designed to investigate the formation and sustainability of empathy-related social closeness (study 1) and examined how sustainably empathy led to prosocial behavior (study 2). Using DDM and fMRI, the last two studies investigated how empathy combined with reciprocity, the social norm to return a favor, on the one hand and empathy combined with the motive of outcome maximization on the other hand altered the behavioral and neural social decision process.
The results showed that empathy-related social closeness and prosocial decision tendencies persisted even if empathy was rarely reinforced. The sustainability of these empathy effects was related to recalibration of the empathy-related social closeness learning signal (study 1) and the maintenance of a prosocial decision bias (study 2). The findings of study 3 showed that empathy boosted the processing of reciprocity-based social decisions, but not vice versa. Study 4 revealed that empathy-related decisions were modulated by the motive of outcome maximization, depending on individual differences in state empathy.
Together, the studies strongly support the concept of empathy as a sustainable driver of social closeness and prosocial behavior.
Social contact is an integral part of daily life. Its health-enhancing effects include reduced negative affective experiences of fear and anxiety, a phenomenon called social buffering. This dissertation studied different forms of social contact and their anxiety-buffering effects with diverse methodologies.
The laboratory-based first study investigated minimal social contact in the context of pain relief learning. Results showed that the observed decreased autonomic and increased subjective fear responses following pain relief learning were independent of social influence. The minimalistic and controlled social setting may have prevented social buffering. Study 2 targeted social buffering in daily life using Ecological Momentary Assessment. We repeatedly assessed individuals’ state anxiety, related cardiovascular responses, and aspects of social interactions with smartphones and portable sensors on five days. Analyses of over 1,500 social contacts revealed gender-specific effects, e.g., heart rate-reducing effects of familiarity in women, but not men. Study 3 examined anxiety, loneliness, and related social factors in the absence of social contact due to social distancing. We constructed and validated a scale measuring state and trait loneliness and isolation, and analysed its link to mental health. Results include a social buffering-like relation of lower anxiety with more trait sociability and sense of belonging.
In sum, the studies showed no fear reduction by minimal social contact, but buffering effects relating to social and personal factors in more complex social situations. Anxiety responses during daily social contacts were lower with more familiar or opposite-gender interaction partners. During limited social contact, lower anxiety related to inter-individual differences in sociability, social belonging, and loneliness. By taking research from lab to life, this dissertation underlined the diverse nature of social contact and its relevance to mental health.
Due to the global aging society and the enormous global incidence and prevalence rates that will result in the coming years, Alzheimer's Dementia (AD) represents a growing challenge for the health care system. The pathogenesis, which is unclear in parts, the chronic progression of AD, which often lasts for years, as well as insufficient diagnostic and therapeutic options complicate an adequate psychotherapeutic and medical approach to the disease. To date, AD is also considered an incurable disease.
Therefore, it is essential to gain deeper insights into the early detection or even prevention of AD. Consideration of prodromal syndromes such as Mild Cognitive Impairment (MCI) can provide significant evidence about high-risk groups for AD progression and differentiate cognitively "normal" aging individuals from those with pathological cognitive decline. Thus, for example, functional Near-Infrared Spectroscopy (fNIRS) imaging helps identify early neurodegenerative processes. In contrast, potential risk factors and predictors of later-onset clinical symptoms of MCI and AD can most often be revealed and quantified via the use of neuropsychiatric test batteries.
The present thesis consists of four studies and aimed to assess and describe the pathological cognitive decline in a sample of elderly study participants (age: ≥ 70 years; N = 604 at baseline) of the longitudinal, observational, and prospective "Vogel Study" from Würzburg, Germany, who were primarily healthy at baseline, over two measurement time points approximately 3 years apart, to differentiate between healthy and diseased study participants and to define predictors of MCI/AD and longitudinal study dropout.
Studies 1 and 2 differentiated healthy study participants from MCI patients based on the baseline hemodynamic response of the parietal cortex recorded by fNIRS during the processing of a paradigm (here: Angle Discrimination Task [ADT]) for visual-spatial processing performance. Neuronal hypoactivity was found in the MCI patients, with both healthy study participants and MCI patients showing higher superior and right hemispheric activation. MCI patients had more difficulty resolving the paradigm. Thus, no evidence of possible compensatory mechanisms was uncovered in the MCI patients.
Study 3 first defined the four latent factors declarative memory, working memory, attention, and visual-spatial processing based on structural equation model (SEM) calculations of the sample using adequate measurement (in-)variant confirmatory factor models from the baseline assessment to the first of a total of two follow-up assessments after approximately 3 years. This allowed a dimensional assessment of pathological cognitive decline versus classificatory-categorical assignment (healthy/diseased) of the sample. In addition, the superiority of the latent factor approach over a composite approach was demonstrated. Next, using a mixed-model approach, predictive analyses were calculated for the prediction of latent factors at first follow-up by baseline risk factors. The sex of study participants proved to be the best predictor of cognitive change in all the cognitive domains, with females performing better than men in the memory domains. Specifically, for declarative memory, older age predicted lower performance regardless of sex. Additional predictive evidence emerged for low serum levels of Brain-Derived Neurotrophic Factor (BDNF) on lower attention performance and higher depression symptoms on lower visual-spatial processing performance.
Study 4 further reported baseline predictors of study dropout at first follow-up. Cognitive performance, as defined in Study 3 using the four latent cognitive factors, was a predictor of study dropout for cognitive decline in the domains of declarative memory, attention, and visual-spatial processing. Conspicuous dementia screening on the Mini-Mental Status Examination (MMSE) also predicted dropout.
Overall, both the use of fNIRS imaging to detect visual-spatial processing performance in the parietal cortex during applying ADT and the dimensional perspective of the neuropsychiatric test battery in the context of prediction and dropout analyses were found to be suitable for early detection research of MCI and AD. Finally, the results will be interpreted in the overall context and implications, limitations, and perspectives will be discussed.
Even though exposure-based cognitive behavioral therapy (CBT) constitutes a first-line treatment for anxiety disorders, a substantial proportion of patients does not respond in a clinically significant manner. The identification of pre-treatment patient characteristics that are associated with treatment outcome might aid in improving response rates. Therefore, the present doctoral thesis aimed at investigating moderators of treatment outcome in anxiety disorders: first, we investigated the neural correlates of comorbidity among primary panic disorder/agoraphobia (PD/AG) and secondary social anxiety disorder (SAD) moderating treatment outcome towards exposure-based CBT. Second, pre-treatment functional resting-state connectivity signatures of treatment response in specific phobia were studied. Within the first study, we compared PD/AG patients with or without secondary SAD regarding their clinical and neurofunctional outcome towards a manualized CBT treatment focusing on PD/AG symptoms. Prior to treatment, PD/AG+SAD compared to PD/AG-SAD patients exhibited a specific neural signature within the temporal lobe, which was attenuated to the level of PD/AG-SAD patients afterwards. CBT was equally effective in both groups. Thus, comorbidity among those two anxiety disorders did not alter treatment outcome substantially. This might be due to the high overlap of shared pathophysiological features within both disorders. In the second study, we assessed pre-treatment functional resting-state connectivity within a sample of spider phobic patients that were treated with massed in virtuo exposure. We found responders already prior to treatment to be characterized by stronger inhibitory frontolimbic connectivity as well as heightened connectivity between the amygdala and regions related to the ventral visual stream. Furthermore, patients demonstrating high within-session extinction exhibited pronounced intrinsic prefrontal connectivity. Our results point to responders exhibiting a brain prepared for the mechanism of action of exposure. Taken together, results highlight the major impact of pre-treatment characteristics on treatment outcome. Both, PD/AG+SAD patients as well as responders within the SpiderVR study exhibited heightened activation or connectivity within the ventral visual pathway and the amygdala. Pronounced visual processing together with enhanced executive control and emotion regulation seem to constitute a fruitful soil for successful exposure. The results provide starting points for personalized treatment approaches in order to improve treatment success in the anxiety disorders. Future studies are needed to investigate the benefit of neuroscientifically informed CBT augmentation strategies such as repetitive transcranial magnetic stimulation.
Background
Performance anxiety is the most frequently reported anxiety disorder among professional musicians. Typical symptoms are - on a physical level - the consequences of an increase in sympathetic tone with cardiac stress, such as acceleration of heartbeat, increase in blood pressure, increased respiratory rate and tremor up to nausea or flush reactions. These symptoms can cause emotional distress, a reduced musical and artistical performance up to an impaired functioning. While anxiety disorders are preferably treated using cognitive-behavioral therapy with exposure, this approach is rather difficult for treating music performance anxiety since the presence of a public or professional jury is required and not easily available. The use of virtual reality (VR) could therefore display an alternative. So far, no therapy studies on music performance anxiety applying virtual reality exposure therapy have investigated the therapy outcome including cardiovascular changes as outcome parameters.
Methods
This mono-center, prospective, randomized and controlled clinical trial has a pre-post design with a follow-up period of 6 months. 46 professional and semi-professional musicians will be recruited and allocated randomly to an VR exposure group or a control group receiving progressive muscle relaxation training. Both groups will be treated over 4 single sessions. Music performance anxiety will be diagnosed based on a clinical interview using ICD-10 and DSM-5 criteria for specific phobia or social anxiety. A behavioral assessment test is conducted three times (pre, post, follow-up) in VR through an audition in a concert hall. Primary outcomes are the changes in music performance anxiety measured by the German Bühnenangstfragebogen and the cardiovascular reactivity reflected by heart rate variability (HRV). Secondary outcomes are changes in blood pressure, stress parameters such as cortisol in the blood and saliva, neuropeptides, and DNA-methylation.
Discussion
The trial investigates the effect of VR exposure in musicians with performance anxiety compared to a relaxation technique on anxiety symptoms and corresponding cardiovascular parameters. We expect a reduction of anxiety but also a consecutive improvement of HRV with cardiovascular protective effects.

Universal prevention for non-suicidal self-injury in adolescents is scarce - A systematic review

Non-suicidal self-injury (NSSI) during adolescence is a high-risk marker for the development and persistence of mental health problems and has been recognized as a significant public health problem. Whereas targeted prevention has indeed shown to be effective in reducing NSSI and improve mental health problems, access to such programs is limited. By face validity, universal prevention of NSSI seems an ideal starting point for a stepped-care model to circumvent a lack of resources in the medical care system. However, it is yet unclear how effective such approaches are. Here, we provide a summary of existing work on universal prevention of NSSI in adolescents younger than 21 years based on a systematic literature search. We found that only seven studies are available. None of the programs evaluated was found to be effective in reducing the incidence or frequency of NSSI. After providing a comprehensive summary of the existing work, we evaluate the fact that existing work primarily focusses on selected/targeted prevention and on psychoeducational methods. We derive implications for future directions in the field of universal prevention of NSSI.
Angsterkrankungen stellen mit einer 12-Monats-Prävalenz von 14% die häufigsten psychischen Erkrankungen in der westlichen Gesellschaft dar. Angesichts der hohen querschnittlichen wie sequentiellen Komorbidität von Angsterkrankungen, der ausgeprägten individuellen Einschränkungen sowie der hohen ökonomischen Belastung für das Gesundheitssystem ist neben therapeutischen Behandlungsansätzen die Entwicklung von kurzzeitigen, kostengünstigen und leicht zugänglichen Präventionsmaßnahmen von großer Bedeutung und steht zunehmend im Fokus des gesundheitspolitischen Interesses, um die Inzidenz von Angsterkrankungen zu reduzieren. Voraussetzung für die Entwicklung von gezielten und damit den effektivsten Präventionsmaßnahmen sind valide Risikofaktoren, die die Entstehung von Angsterkrankungen begünstigen. Ein Konstrukt, das in der Literatur als subklinisches Symptom in Form einer kognitiven Vulnerabilität für Angsterkrankungen und damit als Risikofaktor angesehen wird, ist die sogenannte Angstsensitivität (AS). AS umfasst die individuelle Tendenz, angstbezogene körperliche Symptome generell als bedrohlich einzustufen und mit aversiven Konsequenzen zu assoziieren.
Das Ziel der vorliegenden Arbeit war daher die Etablierung und Validierung eines Präventionsprogramms zur Reduktion der AS an einer nicht-klinischen Stichprobe von 100 Probanden (18-30 Jahre) mit einer erhöhten AS (Anxiety Sensitivity Index [ASI-3] ≥17) sowie die Rekrutierung von 100 alters- und geschlechtsangeglichenen Probanden mit niedriger Angstsensitivität (ASI-3 <17). In einem randomisiert-kontrollierten Studiendesign durchliefen die Probanden mit hoher AS entweder das über fünf Wochen angelegte „Kognitive Angstsensitivitätstraining“ (KAST) als erste deutschsprachige Übersetzung des Computer-basierten „Cognitive Anxiety Sensitivity Treatment“ (CAST) von Schmidt et al. (2014) oder wurden der Wartelisten-Kontrollgruppe zugeteilt. Das KAST Training bestand aus einer einmaligen Vermittlung kognitiv-behavioraler Psychoedukation zum Thema Stress und Anspannung sowie deren Auswirkungen auf den Körper und der Anleitung von zwei interozeptiven Expositionsübungen (‚Strohhalm-Atmung‘ und ‚Hyperventilation‘), die über den anschließenden Zeitraum von fünf Wochen in Form von Hausaufgaben wiederholt wurden.
Es konnte gezeigt werden, dass die Teilnehmer des KAST-Programms nach Beendigung des Trainings (T1) eine signifikant niedrigere AS-Ausprägung im Vergleich zur Wartelisten-Kontrollgruppe aufwiesen und diese Reduktion auch über den Katamnese-Zeitraum von sechs Monaten (T2) stabil blieb. Ergänzend wurde auch die Targetierbarkeit weiterer intermediärer Risikomarker wie der Trennungsangst (TA), des Index der kardialen Sensitivität sowie der Herzratenvariabilität (HRV) untersucht, die jedoch nicht durch das KAST-Training direkt verändert werden konnten. Im Vergleich der Subgruppen von Probanden mit hoher AS und gleichzeitig hoher TA (Adult Separation Anxiety Questionnaire [ASA-27] ≥22) und Probanden mit hoher AS, aber niedriger TA (ASA-27 <22) zeigte sich, dass die AS-TA-Hochrisikogruppe ebenfalls gut von der KAST-Intervention profitieren und eine signifikante Reduktion der AS erzielen konnte, indem sie sich bei T1 dem Niveau der Gruppe mit niedriger TA anglich. Zudem korrelierte die prozentuale Veränderung der Einstiegswerte der inneren Anspannung während der Strohhalm-Atmungsübung positiv mit der prozentualen Veränderung der dimensionalen TA bei T1.
Zusammenfassend weisen die Ergebnisse der vorliegenden Arbeit erstmalig auf die Wirksamkeit der deutschsprachigen Übersetzung des CAST-Programms (Schmidt et al., 2014), eines Computer-basierten, und damit leicht zu implementierenden sowie kostengünstigen Programms, in Bezug auf die Reduktion der AS sowie indirekt der TA hin und können damit zur indizierten und demnach besonders effektiven Prävention von Angsterkrankungen in Hochrisikogruppen beitragen.
‚Social Buffering‘ beschreibt den positiven Einfluss eines Artgenossen auf die Verarbeitung aversiver Reize. In Tierexperimenten zeigte sich, dass Tiere mit geringeren Anspannungsreaktionen reagieren, wenn ein weiteres Tier während der Präsentation von Angstreizen anwesend ist. Eine Untersuchung an einer weiblichen Stichprobe replizierte den Effekt am Menschen. Allerdings gibt es Hinweise auf mögliche Geschlechtsunterschiede. Da vergleichbare Experimente bei Männern fehlen, will sich diese Studie der Frage nähern, ob die reine Anwesenheit einer fremden männlichen Person im Stande ist, autonome Angstreaktionen bei Männern abzumildern.
Dafür wurden 72 männliche, psychisch gesunde Probanden auf zwei Gruppen aufgeteilt, welche eine identische Stimulation mit angstinduzierenden und neutralen Tönen erhielten. Die Männer der Alleinbedingung wurden allein getestet (n allein = 36), die der Sozialbedingung zusammen mit einer fremden männlichen Person (n sozial = 36). Bei allen Probanden wurden die Hautleitfähigkeitsreaktionen (skin conductance response; SCR) während der Antizipation und der Darbietung der Töne erfasst. Außerdem wurden die Probanden nach ihrem Gefühlszustand befragt (Rating). Als relevante Persönlichkeitsdimensionen wurden anhand von Fragebögen die Angstsensitivität (ASI-3), die Ängstlichkeit als Trait (STAI trait), die Ängstlichkeit als State (STAI state) und der Eindruck des Probanden von der anwesenden männlichen Person erhoben.
Die Ergebnisse zeigten keine signifikanten Unterschiede in den SCRs und Ratings bezüglich des angstinduzierenden Tones. Dieses Ergebnis legt nahe, dass bei der männlichen Stichprobe kein ‚Social Buffering‘-Effekt vorlag. Weiterhin waren die autonomen Reaktionen auf die Angstreize höher, je ähnlicher der Mann die fremde Person zu sich bewertete. Die möglichen Ursachen des fehlenden ‚Social-Buffering‘-Effekts werden unter Berücksichtigung von Geschlechtsunterschieden im Umgang mit Angst und sozialer Unterstützung diskutiert.
To slow down the spread of the SARS-Cov-2 virus, countries worldwide severely restricted public and social life. In addition to the physical threat posed by the viral disease (COVID-19), the pandemic also has implications for psychological well-being. Using a small sample (N = 51), we examined how Big Five personality traits relate to coping with contact restrictions during three consecutive weeks in the first wave of the COVID-19 pandemic in Germany. We showed that extraversion was associated with suffering from severe contact restrictions and with benefiting from their relaxation. Individuals with high neuroticism did not show a change in their relatively poor coping with the restrictions over time, whereas conscientious individuals seemed to experience no discomfort and even positive feelings during the period of contact restrictions. Our results support the assumption that neuroticism is a vulnerability factor in relation to psychological wellbeing but also show an influence of contact restrictions on extraverted individuals.
Depressionen und Angststörungen sind die beiden häufigsten psychischen Erkrankungen. Für Angststörungen wurde in zahlreichen Untersuchungen die Bedeutung veränderter Muster in den basalen emotional-assoziativen Lernprozessen für die Ätiologie und Aufrechterhaltung der Erkrankung gezeigt. Hierzu zählen eine verstärkte Akquisitionsreaktion auf den konditionierten Stimulus, Defizite in der Inhibition der Furchtreaktion auf den Sicherheit signalisierenden Stimulus, Übergeneralisierung und Beeinträchtigungen in der Extinktion konditionierter Reaktionen.
Aufgrund der hohen Prävalenzen einer Komorbidität mit Depressionen rückte in den letzten Jahren zunehmend die Untersuchung der genannten Prozesse bei Depressionen in den Fokus. Hierfür konnten bisher keine einheitlichen Ergebnisse gezeigt werden.
Weiterhin wird der Subtyp der ängstlichen Depression einerseits mit hohen Prävalenzen beschrieben, andererseits zeigen Untersuchungen eine schlechtere Prognose, stärkere Einschränkungen in der Funktionalität und ein schlechteres Ansprechen auf die Therapie im Vergleich zu depressiven Patienten ohne hohes Ängstlichkeitsniveau.
In dieser Arbeit wurden die Akquisition, Generalisierung und Extinktion in einem differentiellen Konditionierungsparadigma bei schwer depressiven ängstlichen und nicht ängstlich-depressiven Patienten sowie einer gesunden Kontrollgruppe untersucht. Ängstliche und nicht ängstlich-depressive Patienten zeigten ein beeinträchtigtes Sicherheitslernen in der Akquisition und Beeinträchtigungen in der Extinktion der konditionierten Furcht. Es ergaben sich keine Unterschiede hinsichtlich der Stärke der Generalisierung zwischen Patienten und den gesunden Kontrollen und es konnten keine differenzierenden Muster zwischen den ängstlich- und den nicht ängstlich-depressiven Patienten gezeigt werden.
Zusammenfassend weisen die Ergebnisse auf Veränderungen im Furchtlernen bei Patienten mit Depressionen hin. Es konnten keine Belege für unterschiedliche Mechanismen im Furchtlernen von ängstlich- und nicht ängstlich-depressiven Patienten gefunden werden. Unsere Ergebnisse stützen somit die Klassifikation der ängstlichen Depression als Subtyp der Depression. Weiterhin weisen die Ergebnisse der beeinträchtigten Extinktion bei Patienten mit Depressionen darauf hin, dass Expositionselemente, welche bei der Therapie von Angststörungen als Verfahren der Wahl eingesetzt werden, auch bei der Behandlung von Depressionen integriert werden sollten, um so den Therapieerfolg zu verbessern.
Anxiety patients over-generalize fear, possibly because of an incapacity to discriminate threat and safety signals. Discrimination trainings are promising approaches for reducing such fear over-generalization. Here we investigated the efficacy of a fear-relevant vs. a fear-irrelevant discrimination training on fear generalization and whether the effects are increased with feedback during training. Eighty participants underwent two fear acquisition blocks, during which one face (conditioned stimulus, CS+), but not another face (CS−), was associated with a female scream (unconditioned stimulus, US). During two generalization blocks, both CSs plus four morphs (generalization stimuli, GS1–GS4) were presented. Between these generalization blocks, half of the participants underwent a fear-relevant discrimination training (discrimination between CS+ and the other faces) with or without feedback and the other half a fear-irrelevant discrimination training (discrimination between the width of lines) with or without feedback. US expectancy, arousal, valence ratings, and skin conductance responses (SCR) indicated successful fear acquisition. Importantly, fear-relevant vs. fear-irrelevant discrimination trainings and feedback vs. no feedback reduced generalization as reflected in US expectancy ratings independently from one another. No effects of training condition were found for arousal and valence ratings or SCR. In summary, this is a first indication that fear-relevant discrimination training and feedback can improve the discrimination between threat and safety signals in healthy individuals, at least for learning-related evaluations, but not evaluations of valence or (physiological) arousal.
Purpose
Examine the effects of an 8-week yoga therapy on fatigue in patients with different types of cancer.
Methods
A total of 173 cancer patients suffering from mild to severe fatigue were randomly allocated to yoga intervention (n = 84) (IG) versus waitlist control group (CG) (n = 88). Yoga therapy consisted of eight weekly sessions with 60 min each. The primary outcome was self-reported fatigue symptoms. Secondary outcomes were symptoms of depression and quality of life (QoL). Data were assessed using questionnaires before (T0) and after yoga therapy for IG versus waiting period for CG (T1).
Results
A stronger reduction of general fatigue (P = .033), physical fatigue (P = .048), and depression (P < .001) as well as a stronger increase in QoL (P = .002) was found for patients who attended 7 or 8 sessions compared with controls. Within the yoga group, both higher attendance rate and lower T0-fatigue were significant predictors of lower T1-fatigue (P ≤ .001). Exploratory results revealed that women with breast cancer report a higher reduction of fatigue than women with other types of cancer (P = .016) after yoga therapy.
Conclusion
The findings support the assumption that yoga therapy is useful to reduce cancer-related fatigue, especially for the physical aspects of fatigue. Women with breast cancer seem to benefit most, and higher attendance rate results in greater reduction of fatigue.

Risk Stratification for Bipolar Disorder Using Polygenic Risk Scores Among Young High-Risk Adults

Objective:
Identifying high-risk groups with an increased genetic liability for bipolar disorder (BD) will provide insights into the etiology of BD and contribute to early detection of BD. We used the BD polygenic risk score (PRS) derived from BD genome-wide association studies (GWAS) to explore how such genetic risk manifests in young, high-risk adults. We postulated that BD-PRS would be associated with risk factors for BD.
Methods:
A final sample of 185 young, high-risk German adults (aged 18–35 years) were grouped into three risk groups and compared to a healthy control group (n = 1,100). The risk groups comprised 117 cases with attention deficit hyperactivity disorder (ADHD), 45 with major depressive disorder (MDD), and 23 help-seeking adults with early recognition symptoms [ER: positive family history for BD, (sub)threshold affective symptomatology and/or mood swings, sleeping disorder]. BD-PRS was computed for each participant. Logistic regression models (controlling for sex, age, and the first five ancestry principal components) were used to assess associations of BD-PRS and the high-risk phenotypes.
Results:
We observed an association between BD-PRS and combined risk group status (OR = 1.48, p < 0.001), ADHD diagnosis (OR = 1.32, p = 0.009), MDD diagnosis (OR = 1.96, p < 0.001), and ER group status (OR = 1.7, p = 0.025; not significant after correction for multiple testing) compared to healthy controls.
Conclusions:
In the present study, increased genetic risk for BD was a significant predictor for MDD and ADHD status, but not for ER. These findings support an underlying shared risk for both MDD and BD as well as ADHD and BD. Improving our understanding of the underlying genetic architecture of these phenotypes may aid in early identification and risk stratification.
Unterschiede in Frontaler Kortex Oxygenierung in zweierlei Risikogruppen der Alzheimer Demenz

Die verbesserte medizinische Versorgung führt zu einer zunehmenden Lebenserwartung unserer Gesellschaft. Damit steigt auch die sozioökonomische Relevanz neurodegenerativer Erkrankungen kontinuierlich. Für die Alzheimer Demenz (AD), die dabei die häufigste Ursache darstellt, stehen bisher keine krankheitsmodifizierenden Behandlungsoptionen zur Verfügung. Die lange präklinische Phase der Erkrankung birgt jedoch großes Potential für die Entwicklung neuer Behandlungsoptionen. Das Untersuchen von Risikogruppen ist für die Identifikation von Prädiktoren einer späteren AD Manifestation von besonderem Interesse. In diesem Zusammenhang werden insbesondere das Vorliegen genetischer Risikokonstellationen, wie dem Apolipoprotein E (APOE) Ɛ4-Allel, sowie kognitiver Risikofaktoren, wie der „leichten kognitiven Beeinträchtigung“ (MCI), diskutiert. Die Identifikation präklinischer Aktivierungsunterschiede in relevanten Gehirnregionen von Risikogruppen kann als Basis für die Entwicklung neurofunktioneller Früherkennungs-Marker dienen. Der präfrontale Kortex (PFC), welcher mit der Steuerung von Exekutivfunktionen assoziiert wird, hat sich in diesem Zusammenhang in bisherigen Studien als eine relevante Schlüsselregion manifestiert. Aufgrund der aufwendigen und kostenintensiven bildgebenden Untersuchungsmethoden, sind die genauen Prozesse jedoch noch unklar.
Ziel der vorliegenden Arbeit war es daher, Unterschiede in der PFC Oxygenierung in zweierlei Risikogruppen der AD mit einer kostengünstigeren Bildgebungsmethode, der funktionellen Nahinfrarot Spektroskopie (fNIRS), zu untersuchen. Dafür wurde in einem ersten Schritt, der Trailmaking Test (TMT), ein weitverbreiteter neuropsychologischer Test zur Erfassung exekutiver Funktionen, für fNIRS implementiert. Als Grundlage für die Untersuchung frühpathologischer Prozesse, wurden zunächst gesunde Alterungsprozesse betrachtet. Der Vergleich von jungen und älteren Probanden (n = 20 pro Gruppe) wies neben der Eignung der Testimplementierung für fNIRS auf eine spezifische bilaterale PFC Oxygenierung hin, welche bei jungen Probanden rechtshemisphärisch lateralisiert war. Ältere Probanden hingegen zeigten bei vergleichbaren Verhaltensdaten insgesamt mehr signifikante Kanäle sowie eine Abnahme der Lateralisierung. Dies kann als zusätzlicher Bedarf an Ressourcen in gesunden Alterungsprozessen interpretiert werden.
Im Rahmen der Hauptstudie wurden anschließend insgesamt 604 ältere Probanden im Alter von 70 bis 76 Jahren untersucht. Zunächst wurde die genetische Risikogruppe der Ɛ4-Allel-Träger (n = 78) mit den neutralen Ɛ3-Allel-Trägern (n = 216) und den Trägern des als protektiv geltenden Ɛ2-Allels (n = 50) verglichen. Hierbei zeigte sich eine geringere Oxygenierung der Risikogruppe bei geringer Aufgabenschwierigkeit, während sich ein erhöhter Oxygenierungsanstieg im medialen PFC mit steigender Aufgabenschwierigkeit zeigte. Dies deutet auf einen erhöhten Bedarf an neuronalen Kontrollmechanismen der Risikogruppe zur Bewältigung der steigenden Aufgabenschwierigkeit hin. Die protektive Gruppe zeigte hingegen eine erhöhte Oxygenierung im ventralen PFC mit steigender Aufgabenschwierigkeit, was möglicherweise auf einen präventiven Effekt hindeuten könnte.
Weiterführend wurden MCI-Patienten mit gesunden Probanden (n = 57 pro Gruppe) hinsichtlich des kognitiven Risikofaktors verglichen. Hierbei zeigte sich ein punktuell reduzierter Oxygenierunganstieg der MCI Patienten mit steigender Aufgabenschwierigkeit vor allem im ventralen PFC bei ebenfalls stabiler Verhaltensleistung. Die gefundene Reduktion könnte ein Zeichen für eine aufgebrauchte kognitive Reserve sein, welche Einbußen auf Verhaltensebene voranzugehen scheint.
Diese charakteristischen Unterschiede in den frontalen Oxygenierungsmustern von Risikogruppen (APOE, MCI) könnten als Biomarker zur Früherkennung von AD noch vor dem Auftreten kognitiver Einbußen dienen. Die fNIRS-Untersuchung während der Durchführung des TMT hat sich in diesem Zusammenhang als potentielles Instrument zur Frühdiagnose der präklinischen Phase der AD als geeignet erwiesen. Die Ergebnisse werden unter Einbezug des wissenschaftlichen Kontexts interpretiert und Implikationen für weitere notwendige Studien sowie die klinische Anwendbarkeit diskutiert.
Major depressive disorder and the anxiety disorders are highly prevalent, disabling and moderately heritable. Depression and anxiety are also highly comorbid and have a strong genetic correlation (r(g) approximate to 1). Cognitive behavioural therapy is a leading evidence-based treatment but has variable outcomes. Currently, there are no strong predictors of outcome. Therapygenetics research aims to identify genetic predictors of prognosis following therapy. We performed genome-wide association meta-analyses of symptoms following cognitive behavioural therapy in adults with anxiety disorders (n = 972), adults with major depressive disorder (n = 832) and children with anxiety disorders (n = 920; meta-analysis n = 2724). We (h(SNP)(2)) and polygenic scoring was used to examine genetic associations between therapy outcomes and psychopathology, personality and estimated the variance in therapy outcomes that could be explained by common genetic variants learning. No single nucleotide polymorphisms were strongly associated with treatment outcomes. No significant estimate of h(SNP)(2) could be obtained, suggesting the heritability of therapy outcome is smaller than our analysis was powered to detect. Polygenic scoring failed to detect genetic overlap between therapy outcome and psychopathology, personality or learning. This study is the largest therapygenetics study to date. Results are consistent with previous, similarly powered genome-wide association studies of complex traits.
Jeder Zwanzigste im Alter von über 60 Jahren ist von einer Demenzerkrankung betroffen. Mit zunehmendem Alter steigt der Anteil Betroffener drastisch. Hierbei ist die Alzheimer-Demenz (AD) der häufigste Subtyp der Demenzerkrankungen. Symptomatisch ist diese Erkrankung vorwiegend charakterisiert durch ein Nachlassen der Gedächtnisfunktionen; neuropathologisch weisen Patienten mit AD neurofibrilläre Bündel von Tau-Protein-Ablagerungen, Amyloid-β (Aβ) Plaques sowie einen verringerten zerebralen Blutfluss auf.
Aktuell gibt es noch keine Behandlungsmöglichkeit, um die Erkrankung deutlich zu verlangsamen oder zu stoppen. Bereits Jahrzehnte vor Diagnosestellung der AD beginnen die pathologischen Mechanismen. Aktuelle Behandlungsmethoden setzen jedoch häufig erst nach Diagnosestellung einer AD an, also zu einem Zeitpunkt, an dem das Gehirn schon eine deutliche Neurodegeneration aufweist. Die Untersuchung von Risikogruppen zur Identifikation von frühen Biomarkern und nebenwirkungsarmen Behandlungsmethoden bietet ein großes Potential, um die Erkrankung möglichst früh entdecken und verlangsamen oder vielleicht sogar stoppen zu können. Risikogruppen im späteren Lebensabschnitt sind beispielsweise Träger des genetischen Hauptrisikofaktors Apolipoprotein-E4 (APOE4), Patienten mit einer subjektiven kognitiven Beeinträchtigung sowie Patienten mit einer objektiven leichten kognitiven Beeinträchtigung (engl. mild cognitive impairment; MCI).
Die Untersuchung der hämodynamischen Reaktion mittels funktioneller Nahinfrarotspektroskopie (fNIRS) ist aufgrund der einfachen und kostengünstigen Einsetzbarkeit dieser Methodik besonders praktikabel. Auch der wiederholte Befund einer reduzierten hämodynamischen Reaktion bei Patienten mit AD scheint vielversprechend. Untersuchungen mit AD-Risikogruppen gibt es bisher jedoch nur wenige; zudem weisen diese uneindeutige Befunde auf.
Ziel der vorliegenden Arbeit ist daher die Untersuchung der hämodynamischen Reaktion bei den Risikogruppen ‚APOE4‘ und ‚MCI‘ im Vergleich zu gesunden Kontrollen während Wortflüssigkeitsaufgaben, die mittels fNIRS bereits gut etablierte Aufgaben darstellen. Des Weiteren wird in der vorliegenden Arbeit die Wirkung einer nebenwirkungsarmen Behandlungsmethode im Vergleich zu einer sham-Behandlung bei der Risikogruppe ‚subjektive kognitive Beeinträchtigung‘ untersucht. Bei dieser Behandlungsmethode handelt es sich um ein mittels transkranieller Gleichstromstimulation (engl. transcranial direct current stimulation; tDCS) augmentiertes kognitives Training.
Es zeigt sich für die Risikogruppe APOE4 bei gleicher Leistung im Vergleich zu Trägern anderer Allelvarianten eine verminderte hämodynamische Reaktion im typischerweise aufgabenspezifisch genutzten inferioren frontalen Gyrus. Parallel dazu weist der mediale frontale Gyrus, ein Teil des frontoparietalen Kontrollsystems, eine verstärkte hämodynamische Reaktion auf. Bei der Risikogruppe MCI zeigt sich neben einer schlechteren Testleistung eine verminderte hämodynamische Reaktion des infe-rioren frontotemporalen Kortex, welcher den inferioren frontalen Gyrus umfasst. Das tDCS-augmentierte kognitive Training bewirkt nicht nur einen gruppenunspezifischen Anstieg der hämodynamischen Reaktion im inferioren frontotemporalen Kortex, die tDCS verstärkt diesen Effekt im Vergleich zur sham-Stimulation noch zusätzlich. Dies geht jedoch nicht mit einer Veränderung der Testleistung einher.
Insgesamt deuten die Ergebnisse darauf hin, dass eine reduzierte hämodyna-mische Reaktion bereits in frühen Krankheitsstadien der AD detektierbar ist und dies möglicherweise als Biomarker für eine frühzeitige Detektion und Behandlung genutzt werden könnte. Des Weiteren bietet die tDCS für frühe Krankheitsstadien der AD das Potential einer nebenwirkungsarmen Behandlungsmethode.
Epigenetic mechanisms have been proposed to mediate fear extinction in animal models. Here, MAOA methylation was analyzed via direct sequencing of sodium bisulfite-treated DNA extracted from blood cells before and after a 2-week exposure therapy in a sample of n = 28 female patients with acrophobia as well as in n = 28 matched healthy female controls. Clinical response was measured using the Acrophobia Questionnaire and the Attitude Towards Heights Questionnaire. The functional relevance of altered MAOA methylation was investigated by luciferase-based reporter gene assays. MAOA methylation was found to be significantly decreased in patients with acrophobia compared with healthy controls. Furthermore, MAOA methylation levels were shown to significantly increase after treatment and correlate with treatment response as reflected by decreasing Acrophobia Questionnaire/Attitude Towards Heights Questionnaire scores. Functional analyses revealed decreased reporter gene activity in presence of methylated compared with unmethylated pCpGfree_MAOA reporter gene vector constructs. The present proof-of-concept psychotherapy-epigenetic study for the first time suggests functional MAOA methylation changes as a potential epigenetic correlate of treatment response in acrophobia and fosters further investigation into the notion of epigenetic mechanisms underlying fear extinction.
Most research on human fear conditioning and its generalization has focused on adults whereas only little is known about these processes in children. Direct comparisons between child and adult populations are needed to determine developmental risk markers of fear and anxiety. We compared 267 children and 285 adults in a differential fear conditioning paradigm and generalization test. Skin conductance responses (SCR) and ratings of valence and arousal were obtained to indicate fear learning. Both groups displayed robust and similar differential conditioning on subjective and physiological levels. However, children showed heightened fear generalization compared to adults as indexed by higher arousal ratings and SCR to the generalization stimuli. Results indicate overgeneralization of conditioned fear as a developmental correlate of fear learning. The developmental change from a shallow to a steeper generalization gradient is likely related to the maturation of brain structures that modulate efficient discrimination between danger and (ambiguous) safety cues.
Disorder-relevant but task-unrelated stimuli impair cognitive performance in social anxiety disorder (SAD); however, time course and neural correlates of emotional interference are unknown. The present study investigated time course and neural basis of emotional interference in SAD using event-related functional magnetic resonance imaging (fMRI). Patients with SAD and healthy controls performed an emotional stroop task which allowed examining interference effects on the current and the succeeding trial. Reaction time data showed an emotional interference effect in the current trial, but not the succeeding trial, specifically in SAD. FMRI data showed greater activation in the left amygdala, bilateral insula, medial prefrontal cortex (mPFC), dorsal anterior cingulate cortex (ACC), and left opercular part of the inferior frontal gyrus during emotional interference of the current trial in SAD patients. Furthermore, we found a positive correlation between patients' interference scores and activation in the mPFC, dorsal ACC and left angular/supramarginal gyrus. Taken together, results indicate a network of brain regions comprising amygdala, insula, mPFC, ACC, and areas strongly involved in language processing during the processing of task-unrelated threat in SAD. However, specifically the activation in mPFC, dorsal ACC, and left angular/supramarginal gyrus is associated with the strength of the interference effect, suggesting a cognitive network model of attentional bias in SAD. This probably comprises exceeded allocation of attentional resources to disorder-related information of the presented stimuli and increased self-referential and semantic processing of threat words in SAD.
Introduction
There is mounting evidence for the influence of emotional content on working memory performance. This is particularly important in light of the emotion processing that needs to take place when emotional content interferes with executive functions. In this study, we used emotional words of different valence but with similar arousal levels in an n-back task.
Methods
We examined the effects on activation in the prefrontal cortex by means of functional near-infrared spectroscopy (fNIRS) and on the late positive potential (LPP). FNIRS and LPP data were examined in 30 healthy subjects.
Results
Behavioral results show an influence of valence on the error rate depending on the difficulty of the task: more errors were made when the valence was negative and the task difficult. Brain activation was dependent both on the difficulty of the task and on the valence: negative valence of a word diminished the increase in activation, whereas positive valence did not influence the increase in activation, while difficulty levels increased. The LPP also differentiated between the different valences, and in addition was influenced by the task difficulty, the more difficult the task, the less differentiation could be observed.
Conclusions
Summarized, this study shows the influence of valence on a verbal working memory task. When a word contained a negative valence, the emotional content seemed to take precedence in contrast to words containing a positive valence. Working memory and emotion processing sites seemed to overlap and compete for resources even when words are carriers of the emotional content.
Hintergrund: Das Spielen von Computerspielen ist ein viel diskutiertes Thema. Auf der Suche nach Auswirkungen des Spielens lassen sich einige Studien finden, die Veränderungen im Erleben und Verhalten zeigen [6-8]. Bei der Frage nach der Ursache hierfür, müssen Aspekte wie Persönlichkeit, Hirnphysiologie, neuronale Grundlagen und Genetik untersucht und diskutiert werden. Der bekannte Persönlichkeitsforscher J. Alan Gray beschreibt in seiner Reinforcement Sensitivity Theory (RST) drei verschiedene Hirnsysteme, die das Annäherungs- und Vermeidungsverhalten regulieren. Eines dieser Systeme, das Behavioral Inhibition System (BIS) wird ganz besonders häufig beim Spielen von sogenannten Ego-Shooter Spielen aktiviert.
Ziel: Ziel der Untersuchung war es, herauszufinden, ob das Spielen dieser Computerspiele einen Trainingseffekt auf neurophysiologische Grundlagen der Persönlichkeit, genauer des BIS nach Gray, hat. Wenn es einen erlernten Effekt gibt, müsste ein signifikanter Unterschied zwischen den BIS scores der Spieler und Nicht-Spieler erkennbar sein. Sollte es keinen signifikanten Unterschied geben, kann es dennoch sein, dass sich das neuronale Substrat (also die physiologische Grundlage) des BIS durch das Training verändert, auch wenn dies dann nicht zu einem anderem Verhalten führt.
Methoden: Es wurden die Ergebnisse von jeweils 17 Ego-Shooter-Spielern und Nicht-Spielern aus dem Fragebogen SPSRQ bezüglich Unterschieden in den Mittelwerten der BIS scores mithilfe eines t-Tests miteinander verglichen. Außerdem wurde von allen Probanden fMRT - Datenmaterial gewonnen und zunächst eine zweifaktorielle ANOVA durchgeführt: die Faktoren waren Spieler (Ja/Nein) und BIS score und gemessen wurde die neuronale Aktivität in Amygdala und Hippocampus im resting state. Um den bekannten Störfaktor Genetik miteinzubeziehen, erfolgte anschließend eine dreifaktorielle ANOVA mit der Kovariate TPH2. Für die Bestimmung dieser Kovariate wurde jedem Probanden ein Röhrchen Blut entnommen und eine Genotypisierung durchgeführt.
Ergebnisse: Der Vergleich der BIS scores mittels t-Test liefert keinen signifikanten Unterschied zwischen Spielern und Nicht-Spielern. In der zweifaktoriellen Varianzanalyse zeigen sich signifikante Unterschiede und eine unterschiedliche Richtung der Korrelation. Während die Korrelation von neuronaler Aktivität und BIS score bei den Spielern positiv ist, ist sie bei den Nicht-Spielern negativ. Der Unterschied verliert in der dreifaktoriellen ANOVA mit der Kovariate TPH2 seine Signifikanz.
Schlussfolgerung: Die Ergebnisse der Arbeit führen zu der entscheidenden Frage, wie unterschiedliche neuronale Aktivitäten entstehen. Zwei sehr kontroverse Ansätze stehen sich dabei gegenüber: 1. Der Genotyp hat einen Einfluss auf die neuronale Plastizität während der Entwicklung. Die vorliegende Arbeit, wie auch frühere Studien -wie die von Hahn et al. [58] - geben Hinweise darauf, dass der Genotyp diesen Einfluss besitzt. Die Tendenz, die bei der Genotypisierung zu sehen ist, lässt die Spekulation zu, dass Personen mit einem bestimmten Genotyp eher zu Spielern werden, als Personen mit einer anderen Ausprägung. Um diese Frage zu klären, sind Untersuchungen mit einem größeren Stichprobenumfang notwendig. 2. Die Unterschiede sind das Ergebnis eines sogenannten Trainingseffektes, entstehen also durch Einfluss von außen und hängen ab von den jeweiligen Erlebnissen, die im Laufe eines Lebens gemacht werden. Trotz der Ergebnisse, die den starken Einfluss des Genotyps aufzeigen, bleibt eine Restwahrscheinlichkeit für den Trainingseffekt und der Anreiz für weitere Studien mit dieser Fragestellung.
Regulating our immediate feelings, needs, and urges is a task that we are faced with every day in our lives. The effective regulation of our emotions enables us to adapt to society, to deal with our environment, and to achieve long‐term goals. Deficient emotion regulation, in contrast, is a common characteristic of many psychiatric and neurological conditions. Particularly anxiety disorders and subclinical states of increased anxiety are characterized by a range of behavioral, autonomic, and neural alterations impeding the efficient down‐regulation of acute fear. Established fear network models propose a downstream prefrontal‐amygdala circuit for the control of fear reactions but recent research has shown that there are a range of factors acting on this network. The specific prefrontal cortical networks involved in effective regulation and potential mediators and modulators are still a subject of ongoing research in both the animal and human model. The present research focused on the particular role of different prefrontal cortical regions during the processing of fear‐relevant stimuli in healthy subjects. It is based on four studies, three of them investigating a different potential modulator of prefrontal top‐down function and one directly challenging prefrontal regulatory processes. Summarizing the results of all four studies, it was shown that prefrontal functioning is linked to individual differences in state anxiety, autonomic flexibility, and genetic predisposition. The T risk allele of the neuropeptide S receptor gene, a recently suggested candidate gene for pathologically elevated anxiety, for instance, was associated with decreased prefrontal cortex activation to particularly fear‐relevant stimuli. Furthermore, the way of processing has been found to crucially determine if regulatory processes are engaged at all and it was shown that anxious individuals display generally reduced prefrontal activation but may engage in regulatory processes earlier than non‐anxious subjects. However, active manipulation of prefrontal functioning in healthy subjects did not lead to the typical behavioral and neural patterns observed in anxiety disorder patients suggesting that other subcortical or prefrontal structures can compensate for an activation loss in one specific region. Taken together, the current studies support prevailing theories of the central role of the prefrontal cortex for regulatory processes in response to fear‐eliciting stimuli but point out that there are a range of both individual differences and peculiarities in experimental design that impact on or may even mask potential effects in neuroimaging research on fear regulation.
Die komplexe Pathogenese von Angst und insbesondere der Panikstörung wird sowohl von genetischen Faktoren wie dem Adenosin A2A Rezeptorgen (ADORA2A) 1976T/C Polymorphismus (rs5751876) als auch von neuropsychologischen Faktoren wie einer verzerrten Emotionsverarbeitung und Defiziten in der frühen Informationsverarbeitung beeinflusst. Ziel der vorliegenden doppelblinden, Placebo-kontrollierten Studie war, ein mehrstufiges pathogenetisches Angstmodell zu etablieren, in dem der Einfluss von 300 mg Koffeinzitrat – einem Antagonisten am Adenosin A2A Rezeptor – versus Placebo 1) auf den emotionspotenzierten Startlereflex (negative, neutrale und positive Bilder aus dem International Affective Picture System (IAPS) sowie zusätzlich panikspezifisches Bildmaterial) an 115 gesunden Probanden (m = 57, w = 58) und 2) auf die frühe Informationsverarbeitung (Prepulse-Modifikation (PPM)-Paradigma mit Interstimulus Intervallen (ISI) von 60, 120, 240, 480 und 2000 ms) an vorwiegend derselben Stichprobe von 114 gesunden Probanden (m = 57, w = 57) getestet wurde. Die Probanden wurden dabei für die genetische ADORA2A 1976T/C Variante stratifiziert und mittels des Angstsensitivitäts-Index (ASI) für Angstsensitivität (AS) charakterisiert. Zusätzlich zum erwarteten Haupteffekt der Bildkategorien (höchste Startlemagnituden für negative, niedrigste für positive Bilder) konnte eine Genotyp X Intervention Interaktion auf die Bildkategorien beobachtet werden: Sowohl Trägerschaft des ADORA2A 1976TT Risikogenotyps unter Placebo als auch der Konsum von Koffein bei ADORA2A 1976CC/CT Nicht-Risikogenotypträgerschaft stellten ein Risiko für eine ähnliche, d.h. undifferenzierte physiologische Erregung in Antwort auf negative und neutrale Reize und damit womöglich für eine erhöhte Angstbereitschaft dar. In Übereinstimmung mit dem hypothetisierten multifaktoriellen Risikomodell potenzierte Koffein in Synergie mit dem ADORA2A 1976TT Risikogenotyp die Startlereaktion spezifisch für negative emotionale Reize. Dieser Effekt wurde maßgeblich durch eine hohe Angstsensitivität verursacht. Die höchsten Startlemagnituden nach Koffeineinnahme bei negativen Bildern zeigten sich insbesondere in der weiblichen Stichprobe. Bei panikspezifischen Bildern führte Koffein bei ADORA2A 1976CC/CT Nicht-Risikogenotypträgern dazu, dass im Vergleich zu Placebo weniger zwischen negativen und Panikbildern unterschieden wurde. Bei ADORA2A 1976TT Risikogenotypträgern ergaben sich bzgl. der panikspezifischen Bilder weder unter Koffein- noch unter Placebobedingungen Unterschiede. Bezüglich der frühen Informationsverarbeitung konnte eine Vierfachinteraktion zwischen Genotyp, Intervention, Geschlecht und ISI beobachtet werden. Eine Stratifikation nach ISI ergab, dass die Prepulse Inhibition (PPI) nach Koffeineinnahme für das ISI von 120 ms und von 240 ms bei weiblichen ADORA2A 1976TT Risikogenotypträgern im Vergleich zu männlichen ADORA2A 1976TT Homozygoten eingeschränkt war, während es keine signifikanten Effekte bei ADORA2A 1976CC/CT Nicht-Risikogenotypträgern oder in der Placebogruppe gab. Nur bei hoch ängstlichen Probanden konnte ein signifikanter Interventionseffekt mit verminderter Prepulse Fazilitation (PPF; ISI von 2000 ms) unter Koffein beobachtet werden. Unsere Ergebnisse weisen auf ein komplexes, mehrstufiges und potenziell geschlechtsspezifisches pathogenetisches Angstmodell hin, bei dem genetische und biochemische Faktoren interaktiv das Risiko für defizitäre emotionale Verarbeitungsprozesse und somit möglicherweise auch für Angststörungen erhöhen. Die Ergebnisse zeigen weiterhin, dass weibliche ADORA2A 1976TT Homozygote unter Koffein eine eingeschränkte Fähigkeit haben, irrelevante sensorische Informationen zu filtern, was die Rolle des adenosinergen Systems bei der Pathogenese von Angst zusätzlich stützt. Durch die Definition von multifaktoriellen Risikoprofilen für Angst und insbesondere die Panikstörung, wie in der vorliegenden Arbeit exemplarisch demonstriert, können in Zukunft Fortschritte in der individuellen Primär- und Sekundärprävention erzielt werden.
Background: There is increasing evidence that glial cells play a role in the pathomechanisms of mood disorders and the mode of action of antidepressant drugs.
Methods: To examine whether there is a direct effect on the expression of different genes encoding proteins that have been implicated in the pathophysiology of affective disorders, primary astrocyte cell cultures from rats were treated with two different antidepressant drugs, imipramine and escitalopram, and the RNA expression of brain-derived neurotrophic factor (Bdnf), serotonin transporter (5Htt), dopamine transporter (Dat), and endothelial nitric oxide synthase (Nos3) was examined.
Results: Stimulation of astroglial cell culture with imipramine, a tricyclic antidepressant, led to a significant increase of the Bdnf RNA level whereas treatment with escitalopram did not. In contrast, 5Htt was not differentially expressed after antidepressant treatment. Finally, neither Dat nor Nos3 RNA expression was detected in cultured astrocytes.
Conclusion: These data provide further evidence for a role of astroglial cells in the molecular mechanisms of action of antidepressants.
The etiology of emotion-related disorders such as anxiety or affective disorders is considered to be complex with an interaction of biological and environmental factors. Particular evidence has accumulated for alterations in the dopaminergic and noradrenergic system - partly conferred by catechol-O-methyltransferase (COMT) gene variation - for the adenosinergic system as well as for early life trauma to constitute risk factors for those conditions. Applying a multi-level approach, in a sample of 95 healthy adults, we investigated effects of the functional COMT Val158Met polymorphism, caffeine as an adenosine A2A receptor antagonist (300 mg in a placebo-controlled intervention design) and childhood maltreatment (CTQ) as well as their interaction on the affect-modulated startle response as a neurobiologically founded defensive reflex potentially related to fear- and distress-related disorders. COMT val/val genotype significantly increased startle magnitude in response to unpleasant stimuli, while met/met homozygotes showed a blunted startle response to aversive pictures. Furthermore, significant gene-environment interaction of COMT Val158Met genotype with CTQ was discerned with more maltreatment being associated with higher startle potentiation in val/val subjects but not in met carriers. No main effect of or interaction effects with caffeine were observed. Results indicate a main as well as a GxE effect of the COMT Val158Met variant and childhood maltreatment on the affect-modulated startle reflex, supporting a complex pathogenetic model of the affect-modulated startle reflex as a basic neurobiological defensive reflex potentially related to anxiety and affective disorders.
Rationale
The endocannabinoid (eCB) system is implicated in several psychiatric disorders. Investigating emotional–motivational dysfunctions as underlying mechanisms, a study in humans revealed that in the C385A polymorphism of the fatty acid amide hydrolase (FAAH), the degrading enzyme of the eCB anandamide (AEA), A carriers, who are characterized by increased signaling of AEA as compared to C/C carriers, exhibited reduced brain reactivity towards unpleasant faces and enhanced reactivity towards reward. However, the association of eCB system with emotional–motivational reactivity is complex and bidirectional due to upcoming compensatory processes.
Objectives
Therefore, we further investigated the relationship of the FAAH polymorphism and emotional–motivational reactivity in humans.
Methods
We assessed the affect-modulated startle, and ratings of valence and arousal in response to higher arousing pleasant, neutral, and unpleasant pictures in 67 FAAH C385A C/C carriers and 45 A carriers.
Results
Contrarily to the previous functional MRI study, A carriers compared to C/C carriers exhibited an increased startle potentiation and therefore emotional responsiveness towards unpleasant picture stimuli and reduced startle inhibition indicating reduced emotional reactivity in response to pleasant pictures, while both groups did not differ in ratings of arousal and valence.
Conclusions
Our findings emphasize the bidirectionality and thorough examination of the eCB system’s impact on emotional reactivity as a central endophenotype underlying various psychiatric disorders.
Hintergrund: Die Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung (ADHS) umfasst die klinischen Symptome Hyperaktivität, Impulsivität und Unaufmerksamkeit und besitzt eine große erbliche Komponente. Veränderungen des Dopaminstoffwechsels und des präfrontalen Kortex scheinen mit der Erkrankung assoziiert zu sein. Bekannt sind auch Defizite in exekutiven Funktionen wie Arbeitsgedächtnis und Antworthemmung, welche gemeinsam als ein Endophänotyp der ADHS betrachtet werden. Solche sogenannten intermediären Phänotypen bilden möglicherweise ätiopathogenetisch sinnvollere Untergruppen von Krankheitsbildern als die Unterteilung nach den klassischen Diagnosesystemen. Zahlreiche Untersuchungen zu Arbeitsgedächtnis und Antworthemmung bei ADHS finden Mittelwertsunterschiede im Vergleich zu gesunden Stichproben in behavioralen und hirnphysiologischen Maßen. Ein genetischer Polymorphismus (Val158Met) mit Einfluss auf die Synthese der Catechol-O-Methyltransferase (COMT) moduliert Arbeitsgedächtnis und Antworthemmung bei Gesunden und Patienten mit ADHS. COMT ist ein Enzym, das den Abbau von präfrontalem Dopamin katalysiert. Das Valin(Val)-Allel ist mit einer geringeren, das Methionin(Met)-Allel mit einer höheren Verfügbarkeit von Dopamin in kortikalen Arealen assoziiert. Letzteres scheint die Funktion präfrontaler Hirnareale zu optimieren. Ziel: Es ist unklar, ob die beiden Prozesse Arbeitsgedächtnis und Antworthemmung miteinander in Verbindung stehen, also einen gemeinsamen Endophänotypen bezeichnen, und ob ein möglicher Zusammenhang dieser beiden Funktionen durch Dopamin moduliert wird. Gegenstand der vorliegenden Arbeit ist, (1) die bekannten Mittelwertsunterschiede zwischen Patienten mit ADHS und gesunden Kontrollpersonen zu replizieren, (2) den Zusammenhang von Arbeitsgedächtnis und Antworthemmung mit Korrelationsanalysen zu überprüfen, (3) den Einfluss des COMT-Polymorphismus auf Arbeitsgedächtnis und Antworthemmung zu untersuchen und (4) festzustellen, ob der COMT-Polymorphismus die Stärke der Korrelationen beeinflusst. Die Fragestellungen 3 und 4 sind aufgrund der kleinen Stichproben als explorativ zu bewerten. Methoden: In die Auswertung aufgenommen wurden 45 erwachsene Patienten mit ADHS und 41 nach Alter, Geschlecht, Händigkeit, Intelligenz, Bildungsniveau und Kopfumfang vergleichbare Kontrollen. Arbeitsgedächtnis und Antworthemmung wurden durch eine N-Back- bzw. eine Stoppsignal-Aufgabe operationalisiert. Als abhängige Variablen dienten als behaviorale Maße die Anzahl richtiger Antworten, Reaktionszeiten und Effizienz und als hirnphysiologische Parameter Konzentrationsveränderungen von oxygeniertem (O2Hb) und deoxygeniertem Hämoglobin (HHb) in präfrontalen Arealen, die mittels funktioneller Nah-Infrarot Spektroskopie gemessen wurden. Korrelationen wurden zwischen den Kontrollbedingungen (1-Back und Go-Trials) und zwischen den eigentlichen Bedingungen der Arbeitsgedächtnis- und Stoppsignal-Aufgabe (2-Back und Stopperfolge) berechnet. Ergebnisse: Kontrollen und Patienten zeigten hirnphysiologisch aufgabentypische signifikante Aktivierungsmuster, für das Arbeitsgedächtnis mehr ausgeprägt über dem dorso-lateralen präfrontalen Kortex (DLPFC), für die Antworthemmung mehr über dem inferioren frontalen Kortex (IFC). Fragestellung 1: Patienten zeigten im Vergleich zu den gesunden Kontrollpersonen signifikante Defizite in allen erhobenen behavioralen Maßen. Sie zeigten auch signifikant reduzierte Anstiege von O2Hb über dem linken und rechten IFC für Stopperfolge und eine statistisch tendenziell verringerte O2Hb-Konzentration im linken DLPFC für die 2-Back-Bedingung. Es fanden sich jedoch auch Defizite in den einfachen Kontrollaufgaben (behaviorale Maße und O2Hb während 1-Back). Nach einer Anpassung der Gruppen für die Unterschiede in den Kontrollaufgaben blieben die Gruppenunterschiede in den Aufgaben für Arbeitsgedächtnis und Antworthemmung bestehen. Fragestellung 2: Nur bei den Patienten und nur für die behavioralen Maße zeigten sich signifikante positive Assoziationen. Die Effizienz während 1-Back korrelierte mit der Reaktionszeit während der Go-Trials, und die Effizienz während 2-Back korrelierte mit der Stoppsignal-Reaktionszeit. Diese beiden Korrelationen unterschieden sich statistisch nicht in ihrer Stärke. In den für die Unterschiede in den Kontrollaufgaben angepassten Gruppen blieb nur die Korrelation der Kontrollaufgaben tendenziell signifikant, die Korrelation von Effizienz während 2-Back und Stoppsignal-Reaktionszeit verschwand. Die Patienten zeigten erhöhte Impulsivität (erhoben mit einem Fragebogen), die positiv mit behavioralen Maßen der beiden untersuchten Prozesse korrelierte. Fragestellung 3: Für 1-Back (O2Hb in rechter Hemisphäre) und 2-Back (O2Hb in linker Hemisphäre) wurden statistisch tendenzielle COMT x Gruppe Interaktionen gefunden. Für 1-Back zeigten die Kontrollen mit dem Val/Val-Genoytp sowohl im Vergleich zu Kontrollen mit dem Met/Met-Genotyp als auch im Vergleich zu Patienten mit zwei Val-Allelen eine signifikant höhere O2Hb-Konzentration. Gleichzeitig wies die Analyse der Verhaltensdaten auf eine höhere Effizienz der Kontrollen mit Met-Allelen im Vergleich zu Patienten mit Met-Allelen hin. Für 2-Back zeigte der Val/Val-Genotyp der Kontrollen eine höhere O2Hb-Konzentration als der Val/Val-Genotyp der Patienten. Zusätzlich wies der Met/Met-Genotyp in der ADHS-Gruppe einen stärkeren Anstieg von O2Hb im Vergleich zu dem Val/Val-Genotyp in der ADHS-Gruppe auf. Die Analyse der Verhaltensdaten zeigte eine höhere Effizienz der Kontrollen im Vergleich zu den Patienten für die Gruppen mit einem oder zwei Val-Allelen. Für Stopperfolge zeigten sich signifikante COMT x Gruppe Interaktionen für O2Hb in beiden Hemisphären. Die Kontrollen mit Val/Met-Genotyp hatten höhere O2Hb-Konzentrationen als Kontrollen mit Val/Val- oder Met/Met-Genotyp und als Patienten mit Val/Met-Genotyp. Patienten mit Val/Met-Genotyp hatten langsamere Stoppsignal-Reaktionszeiten im Vergleich zu den Kontrollen mit Val/Met-Genotyp. Fragestellung 4: Die Korrelationen in Abhängigkeit der Stichproben und COMT-Gruppen zeigten unterschiedlich hohe und unterschiedlich gerichtete Korrelationskoeffizienten. Nur in der Gruppe der Patienten und nur für die Verhaltensdaten wurden positive Assoziationen für alle COMT-Gruppen gefunden, vergleichbar mit den Ergebnissen aus Fragestellung 2. Diskussion: Die behavioralen und hirnphysiologischen Unterschiede in den Aufgaben zum Arbeitsgedächtnis und zur Antworthemmung zwischen Patienten mit ADHS und Kontrollen replizieren gut die Ergebnisse früherer Arbeiten. Ausgehend von den Gruppenunterschieden in den einfachen Kontrollaufgaben muss aber überlegt werden, inwieweit Patienten mit ADHS auch in einfachen psychomotorischen Aufgaben bereits Defizite zeigen. Die Korrelation behavioraler Maße der eigentlichen Arbeitsgedächtnis- und Inhibitionsaufgabe in der Gruppe der Patienten, die allerdings die Assoziation der Kontrollaufgaben nicht überschritt, und die mit dem Anpassen für Unterschiede in den Kontrollenaufgaben verschwand, lässt Zweifel an dem spezifischen Zusammenhang der beiden Prozesse bei ADHS aufkommen. Zudem werfen die positiven Assoziationen dieser Prozesse (behaviorale Maße) mit der Impulsivität (Fragebogen) die Frage auf, ob es sich um die Operationalisierung des gleichen Konstrukts handelt bzw. ob Impulsivität in Form behavioraler Tests das Gleiche misst wie Impulsivität auf symptomatischer Ebene. Ein Faktor, der möglicherweise Einfluss auf die Korrelationen zwischen den Aufgaben hat, könnte der Dopamingehalt sein, wie er in der vorliegenden Arbeit durch den COMT Polymorphismus operationalisiert wurde. Die Ergebnisse der COMT x Gruppe Interaktionen zeigten eine erhöhte O2Hb-Konzentration der Val-Allelträger der Kontrollen für die N-Back-Aufgaben bei teilweise besserer Effizienz und eine erhöhte O2Hb-Konzentration und teilweise schnellere Stoppsignal-Reaktionszeit der Val/Met-Allelträger der Kontrollen bei Stopperfolgen. Unsere Ergebnisse entsprechen nicht unbedingt den Annahmen aus der Literatur, dass das Met-Allel das günstige Allel in Bezug auf gute präfrontale Hirnfunktion sei. Diese Unterschiede in den COMT x Gruppe Interaktionen legen eine Abhängigkeit präfrontaler Hirnfunktionen vom allgemeinen dopaminergen Gehalt (Patienten vs. Kontrollen), vom präfrontalen dopaminergen Niveau (COMT-Genotyp) und von der Aufgabenart (Arbeitsgedächtnis vs. Antworthemmung) nahe. Diese Hypothese wird unterstützt durch die Unterschiede in den Korrelationskoeffizienten der beiden Prozesse in Abhängigkeit der Stichproben und Genotyp-Gruppen. Schlussfolgerung: Die Befunde stützen nicht die Annahme, dass es sich bei Arbeitsgedächtnis und Antworthemmung um zusammenhängende Funktionen bei ADHS oder Gesunden handelt, oder dass der exekutive Endophänotyp einen einheitlichen Subtyp der ADHS darstellt. Zudem scheint eine mögliche Assoziation der beiden Prozesse von mehreren Faktoren wie z. B. der Dopamin-Konzentration moduliert zu werden. Zukünftige Arbeiten sollten die Fragestellungen bearbeiten, welche Faktoren den Zusammenhang dieser Prozesse beeinflussen und welche Kombination solcher Faktoren eine Subgruppe von Patienten mit ADHS mit einem gemeinsamen exekutiven Endophänotyp darstellen könnten.
The maximum of the brain electrical field after NoGo stimuli is located more anteriorly than that after stimuli that tells participants to respond. The difference in topography was called NoGo-Anteriorization (NGA). Recently, there was a debate, whether the NGA is related to a central inhibitory process or not. However, experiments showed that the NGA is not the result of motor potentials during Go trials, the NGA does not represent higher response conflict and or higher mental effort in NoGo trials, and the NGA is not based on less cognitive response selection in NoGo trials. Therefore, the experiments support the assumption that the NGA is connected to an inhibitory mechanism in NoGo conditions.
Low P300 amplitudes and topographical asymmetries have been reponed in schizophrenic patients, but reference-independent amplitude assessment failed to replicate reduced amplitudes. P300 amplitude is conventially assessed at midline electrodes (PZ), anti asymmetric topography as reported in schizophrenics, may conj'ound this measurement. We lnvestigated the possible Interaction between P300 ropography and assessments of amplitudes. ln 41 clinically stable schizophrenics and 31 normal controls, the generalfinding ofreduced amplitudes at the P'l electrode and topographical asymmetrles in the patient group were replicated. ln both groups, a.symmetries of the P300 field (lateralized peaks) reduced the standard amplitude assessment at the midline parletal electrode, but did not Qjfoct the reference-independent, global amplitude assessment. This shows thal asymmetry per se does not imply reduced field strength. in addition, in schizophreraics. but not in controls, there was a significcmt effect oftlae direction of asymmetry on both amplltude measures, amplitudes belng lower with increasing shift ofthe P300 peak to the right side. Considering also the slightly left-lateralized peaks in the normal controls. this suggests rhat only right lateralized P300 peaks upressfunctional deficits in schizophrenics, whereas left lateralized pealcs fall wlthin the physiological variability of the P3OO field. Tht refonnce-independent amplitude assessment is proposed for unambiguous amplitude assessment in order to better define the clinical, psychological and physiopathological mtaning of the P3OO alterations in schizophrenics.
Clinical evidence for a dominant mode of inheritance and anticipation in periodic catatonia, a distinct subtype of schizophrenia, indicates that genes with triplet repeat expansions or other unstable repetitive elements affecting gene expression may be involved in the etiology of this disorder. Because patients affected with dentatorubral-pallidoluysian atrophy (DRPLA) may present with "schizophrenic" symptoms, we have investigated the DRPLA (B 37 CAG repeat) locus on chromosome 12 in 41 patients with periodic catatonia. The B 37 CAG repeat locus was highly polymorphic but all alleles in both the patient and control group had repeat sizes within the normal range. We conclude that variation at the DRPLA locus is unlikely to be associated with periodic catatonia. The evidence for dominant inheritance and anticipation as well as the high prevalence of human brain genes containing trinucleotide repeats justifies further screening for triplet repeat expansions in periodic catatonia.
Evidence of hemispheric asymmetries in schizophrenia has been reported from different research areas. Asymmetries in evoked potential P300 topography are still controversial because of inconsistent findings. In the present study. previous results of abnormal lateralization of P300 were replicated in stabilized residual Schizophrenie patients. Auditory P300 was recorded during an odd ball task in which subjeets detected rare target stimuli. Schizophrenie patients had the P300 peak shifted to the right hemisphere and differed signifieantly from age- and sex-matched normal control subjects who had left-lateralized P300 peaks. A comparison of different methods of assessment and analysis of the topographical features of the P300 electric fields showed that the extraction of reference-independent descriptors of P300 topography is a reliable and sensitive method for statistical handling of the maps. The results suggest left hemispheric dysfunction during cognitive tasks in a subgroup of Schizophrenie patients. Inconsistencies between previous sturlies are likely to be due to heterogeneous patient groups, which may have included patients in an acute Schizophrenie episode or patients in clinical remission. lnvestigation of the clinical meaning of P300 alterations requires careful psychopathological definition of the patient groups.
In a retrospective study of 80 chronic DSM 111-R schizophrenics and 80 controls, the occurrence of obstetric complications (OCs) into the development of chronic schizophrenias was investigated using Leonhard s distinction in systematic schizophrenia (no obvious familial loading) and unsystematic schizophrenia (mainly genetically determined according to Leonhard). The Lewis & Murray and Fuchs scales were used for evaluation. In both scales, unsystematic schizophrenias did not differ from controls, but those with OCs were significantly (p < 0.01) earlier hospitalized (20.5 years) than those without OCs (25.6 years). Systematic schizophrenics had an increased frequency, severity and total score of OCs compared to controls in the Fuchs scale (p < 0.0 I). Likewise, in the Lewis & Murray scale systematic schizophrenia showed an increased presence ofOCs compared to controls (p < 0.05) and to unsystematic schizophrenia (p < 0.1 ). Systematic schizophrenias were significantly allocated to matemal infectious diseases during mid-gestation. Patients with matemal infections showed moreadditional OCs than those without (p < 0.05; Lewis & Murray scale). In systematic schizophrenia, a history of OC was not associated with an early onset of the disease. In the genetic determined schizophrenias prenatal and perinatal disturbanccs Iead to an early onset of the disease, however, in systematic schizophrenias they seem to be of causal importance for the development of the disease.
