Objectives
Specific serological tests are mandatory for reliable SARS‐CoV‐2 diagnostics and seroprevalence studies. Here, we assess the specificities of four commercially available SARS‐CoV‐2 IgG ELISAs in serum/plasma panels originating from Africa, South America, and Europe.
Methods
882 serum/plasma samples collected from symptom‐free donors before the COVID‐19 pandemic in three African countries (Ghana, Madagascar, Nigeria), Colombia, and Germany were analysed with three nucleocapsid‐based ELISAs (Euroimmun Anti‐SARS‐CoV‐2‐NCP IgG, EDI™ Novel Coronavirus COVID‐19 IgG, Mikrogen recomWell SARS‐CoV‐2 IgG), one spike/S1‐based ELISA (Euroimmun Anti‐SARS‐CoV‐2 IgG), and in‐house common cold CoV ELISAs.
Results
High specificity was confirmed for all SARS‐CoV‐2 IgG ELISAs for Madagascan (93.4–99.4%), Colombian (97.8–100.0%), and German (95.9–100.0%) samples. In contrast, specificity was much lower for the Ghanaian and Nigerian serum panels (Ghana: NCP‐based assays 77.7–89.7%, spike/S1‐based assay 94.3%; Nigeria: NCP‐based assays 39.3–82.7%, spike/S1‐based assay 90.7%). 15 of 600 African sera were concordantly classified as positive in both the NCP‐based and the spike/S1‐based Euroimmun ELISA, but did not inhibit spike/ACE2 binding in a surrogate virus neutralisation test. IgG antibodies elicited by previous infections with common cold CoVs were found in all sample panels, including those from Madagascar, Colombia, and Germany and thus do not inevitably hamper assay specificity. Nevertheless, high levels of IgG antibodies interacting with OC43 NCP were found in all 15 SARS‐CoV‐2 NCP/spike/S1 ELISA positive sera.
Conclusions
Depending on the chosen antigen and assay protocol, SARS‐CoV‐2 IgG ELISA specificity may be significantly reduced in certain populations probably due to interference of immune responses to endemic pathogens like other viruses or parasites.
Analysis of structural brain asymmetries in attention‐deficit/hyperactivity disorder in 39 datasets

Objective
Some studies have suggested alterations of structural brain asymmetry in attention‐deficit/hyperactivity disorder (ADHD), but findings have been contradictory and based on small samples. Here, we performed the largest ever analysis of brain left‐right asymmetry in ADHD, using 39 datasets of the ENIGMA consortium.
Methods
We analyzed asymmetry of subcortical and cerebral cortical structures in up to 1,933 people with ADHD and 1,829 unaffected controls. Asymmetry Indexes (AIs) were calculated per participant for each bilaterally paired measure, and linear mixed effects modeling was applied separately in children, adolescents, adults, and the total sample, to test exhaustively for potential associations of ADHD with structural brain asymmetries.
Results
There was no evidence for altered caudate nucleus asymmetry in ADHD, in contrast to prior literature. In children, there was less rightward asymmetry of the total hemispheric surface area compared to controls (t = 2.1, p = .04). Lower rightward asymmetry of medial orbitofrontal cortex surface area in ADHD (t = 2.7, p = .01) was similar to a recent finding for autism spectrum disorder. There were also some differences in cortical thickness asymmetry across age groups. In adults with ADHD, globus pallidus asymmetry was altered compared to those without ADHD. However, all effects were small (Cohen’s d from −0.18 to 0.18) and would not survive study‐wide correction for multiple testing.
Conclusion
Prior studies of altered structural brain asymmetry in ADHD were likely underpowered to detect the small effects reported here. Altered structural asymmetry is unlikely to provide a useful biomarker for ADHD, but may provide neurobiological insights into the trait.
Purpose
High myopic patients may develop strabismus due to globe dislocation out of the normal extraocular muscle cone. Surgical correction of this strabismus type is possible by joining the superior and lateral rectus muscles without the need for a scleral suture called the Yokoyama procedure. Data from large patient samples and the evaluation of a potential effect of an additional medial rectus recession (MRR) have been lacking so far.
Methods
We pooled retrospective patient data of 14 departments of ophthalmology in Germany and Switzerland and analysed determinants of postoperative results using multivariable regression models.
Results
We included 133 patients (mean age: 59.7 ± 13.4 years, surgery between 2008 and 2017) with a mean preoperative esotropia (both Yokoyama with and without MRR) of 23.8°±4.6°. The angle of preoperative esotropia increased with age. The postoperative esotropia was 8.7° ± 9.9°, and six patients were overcorrected. While preoperative esotropia was highly associated with postoperative results, we found no association of additional MRR with any of our postoperative outcome measures. The Yokoyama procedure had a higher absolute effect in patients with higher preoperative esotropia.
Conclusion
Our study confirms the positive effect of the Yokoyama procedure on strabismus due to high myopia in large‐scale real‐world data. In some cases, MRR may be needed because of muscle contracture, although additional MRR statistically did not affect the postoperative outcome. In patients with bilateral high myopic strabismus, correction of both eyes seems beneficial. The effect size of the Yokoyama procedure appears to be mainly driven by preoperative esotropia.
Aminoacyl‐tRNA synthetases (ARSs) catalyze the first step of protein biosynthesis (canonical function) and have additional (non‐canonical) functions outside of translation. Bi‐allelic pathogenic variants in genes encoding ARSs are associated with various recessive mitochondrial and multisystem disorders. We describe here a multisystem clinical phenotype based on bi‐allelic mutations in the two genes (FARSA, FARSB) encoding distinct subunits for tetrameric cytosolic phenylalanyl‐tRNA synthetase (FARS1). Interstitial lung disease with cholesterol pneumonitis on histology emerged as an early characteristic feature and significantly determined disease burden. Additional clinical characteristics of the patients included neurological findings, liver dysfunction, and connective tissue, muscular and vascular abnormalities. Structural modeling of newly identified missense mutations in the alpha subunit of FARS1, FARSA, showed exclusive mapping to the enzyme's conserved catalytic domain. Patient‐derived mutant cells displayed compromised aminoacylation activity in two cases, while remaining unaffected in another. Collectively, these findings expand current knowledge about the human ARS disease spectrum and support a loss of canonical and non‐canonical function in FARS1‐associated recessive disease.
In deafness, which is caused by the malfunctioning of the inner ear, an implantation of a cochlear implant (CI) is able to restore hearing. The CI is a neural prosthesis that is located within the cochlea. It replaces the function of the inner hair cells by direct electrical stimulation of the auditory nerve fibers. The CI enables many deaf or severe hearing-impaired people to achieve a good speech perception. Nevertheless, there is a lot of potential for further improvements. Compared to normal-hearing listeners rate pitch discrimination is much worse. Rate pitch discrimination is the ability to distinguish the pitch of two stimuli with two different pulse rates. This ability is important for enjoying music as well as speech perception (in noise). Further, the small dynamic range in electrical hearing (compared to normal-hearing listeners) and therefore the small intensity resolution limits the performance of CI users. Both, rate pitch coding and dynamic range were investigated in this doctoral thesis.
For the first issue, a pitch discrimination task was designed to determine the just-noticeable-difference (JND) in pitch with 200 and 400 pps as reference. Additionally to the default biphasic pulse (single pulse) the experiment was performed with double pulses. The double pulse consists out of two biphasic pulses directly after each other and a small interpulse interval (IPI) in between. Three different IPIs (15, 50, and 150 µs) were tested. The statistical analysis of JNDs revealed no significant effects between stimulation with single-pulse or double-pulse trains.
A follow-up study investigated an alternating pulse train consisting of single and double pulses. To investigate if the 400 pps alternating pulse train is comparable in pitch with the 400 pps single-pulse train, a pairwise pitch comparison test was conducted. The alternating pulse train was compared with single-pulse trains at 200, 300 and 400 pps. The results showed that the alternating pulse train is for most subjects similar in pitch with the 200 pps single-pulse train. Therefore, pitch perception seemed to be dominated by the double pulses within the pulse train.
Accordingly, double pulses with different amplitudes were tested. Based on the facilitation effect, a larger neuronal response was expected by stimulating with two pulses with a short IPI within the temporal facilitation range. In other studies, this effect was shown to be maximal in CIs of the manufacturer Cochlear, with first pulse amplitudes set at or slightly below the electrically evoked compound action potential (ECAP) threshold. The second pulse amplitude did not influence the facilitation effect and therefore could be choose at will. Similarly, this effect was tested in this thesis with CIs of the manufacturer MED-EL. Nevertheless, to achieve a proper signal-to-noise ratio, technical issues had to be addressed like a high noise floor, resulting in incorrect determination of the ECAP threshold. After solving this issues, the maximum facilitation effect was around the ECAP threshold as in the previous study with Cochlear. For future studies this effect could be used in a modified double pulse rate pitch experiment with the first pulse amplitude at ECAP threshold and the second pulse amplitude variable to set the most comfortable loudness level (MCL).
The last study within this thesis investigated the loudness perception at two different loudness levels and the resulting dynamic range for different interphase-gaps (IPG). A larger IPG can reduce the amplitude at same loudness level to save battery power. However, it was unknown if the IPG has an influence on the dynamic range. Different IPGs (10 and 30 µs) were compared with the default IPG (2.1 µs) in a loudness matching experiment. The experiment was performed at the most comfortable loudness level (MCL) of the subject and the amplitude of half the dynamic range (50%-ADR). An upper dynamic range was calculated from the results of MCL and 50%-ADR (therefore not the whole dynamic range was covered). As expected from previous studies a larger IPG resulted in smaller amplitudes. However, the observed effect was larger at MCL than at 50%-ADR which resulted in a smaller upper dynamic range. This is the first time a decrease of this dynamic range was shown.
Design of novel IL-4 antagonists employing site-specific chemical and biosynthetic glycosylation

The cytokines interleukin 4 (IL-4) and IL-13 are important mediators in the humoral immune response and play a crucial role in the pathogenesis of chronic inflammatory diseases, such as asthma, allergies, and atopic dermatitis. Hence, IL-4 and IL-13 are key targets for treatment of such atopic diseases.
For cell signalling IL-4 can use two transmembrane receptor assemblies, the type I receptor consisting of receptors IL-4R and γc, and type II receptor consisting of receptors IL-4R and IL-13R1. The type II receptor is also the functional receptor of IL-13, receptor sharing being the molecular basis for the partially overlapping effects of IL-4 and IL-13. Since both cytokines require the IL-4R receptor for signal transduction, this allows the dual inhibition of both IL-4 and IL-13 by specifically blocking the receptor IL-4R.
This study describes the design and synthesis of novel antagonistic variants of human IL-4. Chemical modification was used to target positions localized in IL-4 binding sites for γc and IL-13R1 but outside of the binding epitope for IL-4R. In contrast to existing studies, which used synthetic chemical compounds like polyethylene glycol for modification of IL-4, we employed glycan molecules as a natural alternative. Since glycosylation can improve important pharmacological parameters of protein therapeutics, such as immunogenicity and serum half-life, the introduced glycan molecules thus would not only confer a steric hindrance based inhibitory effect but simultaneously might improve the pharmacokinetic profile of the IL-4 antagonist.
For chemical conjugation of glycan molecules, IL-4 variants containing additional cysteine residues were produced employing prokaryotic, as well as eukaryotic expression systems. The thiol-groups of the engineered cysteines thereby allow highly specific modification. Different strategies were developed enabling site-directed coupling of amine- or thiol- functionalized monosaccharides to introduced cysteine residues in IL-4. A linker-based coupling procedure and an approach requiring phenylselenyl bromide activation of IL-4 thiol-groups were hampered by several drawbacks, limiting their feasibility. Surprisingly, a third strategy, which involved refolding of IL-4 cysteine variants in the presence of thiol- glycans, readily allowed synthesis of IL-4 glycoconjugates in form of mixed disulphides in milligram amount. This approach, therefore, has the potential for large-scale synthesis of IL-4 antagonists with highly defined glycosylation. Obtaining a homogenous glycoconjugate with exactly defined glycan pattern would allow using the attached glycan structures for fine-tuning of pharmacokinetic properties of the IL-4 antagonist, such as absorption and metabolic stability.
The IL-4 glycoconjugates generated in this work proved to be highly effective antagonists inhibiting IL-4 and/or IL-13 dependent responses in cell-based experiments and in in vitro binding studies. Glycoengineered IL-4 antagonists thus present valuable alternatives to IL-4 inhibitors used for treatment of atopic diseases such as the neutralizing anti-IL-4R antibody Dupilumab.
Der hepatische Ischämie-Reperfusionsschaden stellt ein großes Problem in der Transplantations- und Leberchirurgie dar: Insbesondere durch Fibrose, Steatose oder Entzündungsprozesse vorgeschädigte Organe zeigen eine erhöhte Vulnerabilität für den Reperfusionsschaden. Protektive Effekte einer Therapie mit mesenchymalen Stammzellen konnten bereits in Vorversuchen gezeigt werden. Ein direkter Vergleich mit den morphologisch sehr ähnlichen Fibroblasten wurde bisher nicht durchgeführt. Diese Wirkung scheint nach aktuellem Forschungsstand nicht durch zellgebundene, sondern parakrine Effekte vermittelt zu werden. Eine präemptive Injektion von Extrazellulärvesikel aus dem Überstand von Zellkulturen zeigte ähnliche Effekte wie eine Therapie mit Stammzellen. Das in dieser Arbeit durchgeführte Tierversuchsmodell basiert auf einer chirurgisch induzierten 70% Ischämie der Mausleber mit präemptiver Injektion von mesenchymalen Stammzellen, Fibroblasten, sowie deren jeweilige Extrazellulärvesikel. Eine präemptive Therapie mit mesenchymalen Stammzellen und deren Extrazellulärvesikeln verringerte den Leberzellschaden, gemessen anhand der Serumtransaminasenspiegel und Ausprägung der Nekrosefläche innerhalb Ischämie-exponierter Leberabschnitte, und konnte die Leberzellregeneration durch vermehrte Ausbildung von Lipid-Microdroplets und erhöhte Zellproliferationsraten der Hepatozyten bis in die Spätphase des Ischämie-Reperfusionsschadens beschleunigen. In Tieren mit einer präemptiven Injektion von Fibroblasten und deren Extrazellulärvesikel konnten diese Effekte nicht nachgewiesen werden. Es konnte kein Unterschied zwischen einer Therapie mit mesenchymalen Stammzellen und deren Extrazellulärvesikeln festgestellt werden.
The incidence of melanoma and nonmelanoma skin cancer has increased tremendously in recent years. Although novel treatment options have significantly improved patient outcomes, the prognosis for most patients with an advanced disease remains dismal. It is, thus, imperative to understand the molecular mechanisms involved in skin carcinogenesis in order to develop new targeted treatment strategies. Receptor tyrosine kinases (RTK) like the ERBB receptor family, including EGFR/ERBB1, ERBB2/NEU, ERBB3, and ERBB4, are important regulators of skin homeostasis and their dysregulation often results in cancer, which makes them attractive therapeutic targets. Members of the leucine‐rich repeats and immunoglobulin‐like domains protein family (LRIG1‐3) are ERBB regulators and thus potential therapeutic targets to manipulate ERBB receptors. Here, we analyzed the function of LRIG1 during chemically induced skin carcinogenesis in transgenic mice expressing LRIG1 in the skin under the control of the keratin 5 promoter (LRIG1‐TG mice). We observed a significant induction of melanocytic tumor formation in LRIG1‐TG mice and no difference in papilloma incidence between LRIG1‐TG and control mice. Our findings also revealed that LRIG1 affects ERBB signaling via decreased phosphorylation of EGFR and increased activation of the oncoprotein ERBB2 during skin carcinogenesis. The epidermal proliferation rate was significantly decreased during epidermal tumorigenesis under LRIG1 overexpression, and the apoptosis marker cleaved caspase 3 was significantly activated in the epidermis of transgenic LRIG1 mice. Additionally, we detected LRIG1 expression in human cutaneous squamous cell carcinoma and melanoma samples. Therefore, we depleted LRIG1 in human melanoma cells (A375) by CRISPR/Cas9 technology and found that this caused EGFR and ERBB3 downregulation in A375 LRIG1 knockout cells 6 h following stimulation with EGF. In conclusion, our study demonstrated that LRIG1‐TG mice develop melanocytic skin tumors during chemical skin carcinogenesis and a deletion of LRIG1 in human melanoma cells reduces EGFR and ERBB3 expression after EGF stimulation.
Bei seiner Geburt und innerhalb der ersten Lebensmonate ist der Säuglingsschädel verhältnismäßig leicht verformbar. Dies birgt die Gefahr einer unphysiologischen Verformung durch externe modellierende Kräfte. Die auf diesem Weg am häufigsten verursachte Deformation ist der Lagerungsplagiozephalus (LP).
In der vorliegenden Studie wurden 455 Säuglinge, die zunächst in drei unterschiedliche Gruppen bezüglich ihrer Kopfform unterteilt worden sind, hinsichtlich verschiedener Parameter miteinander verglichen.
Anhand des U-Heftes und einem speziell für die craniofaciale Sprechstunde des CFCW Würzburg angefertigten Fragebogen wurden Prädiktoren für die Entwicklung eines LP evaluiert. Die herausgearbeiteten prädisponierenden Faktoren waren das männliche Geschlecht, Frühgeburtlichkeit, eine unphysiologische Geburtslage, Notkaiserschnitt oder geburtshilfliche Maßnahmen, verminderte Geburtsgröße, vermindertes Geburtsgewicht und ein längerer Krankenhausaufenthalt im Anschluss an die Geburt. Als prognostisch günstiger Faktor hinsichtlich der Entwicklung einer physiologischen Schädelform konnte in der vorliegenden Studie eine längere Stilldauer bestätigt werden. Dies galt ebenso für Gabe von Flaschennahrung aus alternierenden Positionen.
Hinsichtlich der präventiven Aufklärung von Eltern Neugeborener liefert die vorliegende Studie einige wichtige Ansätze. Sie untermauert jedoch auch den hohen Bedarf an weiterer Forschung bezüglich prädisponierender Faktoren für die Entwicklung des LP. Dies kann dazu beitragen die Prävention und Früherkennung eines LP mittels flächendeckender qualitativ hochwertiger Aufklärung stetig zu verbessern und notwendig gewordene Behandlungen durch standardisierte Therapieempfehlungen zu optimieren.
Anamnestic screening of symptoms and contact history is applied to identify coronavirus disease 2019 (COVID‐19) patients on admission. However, asymptomatic and presymptomatic patients remain undetected although the viral load may be high. In this retrospective cohort study, all hospitalized patients who received polymerase chain reaction (PCR) admission testing from March 26th until May 24th, 2020 were included. Data on COVID‐19‐specific symptoms and contact history to COVID‐19 cases were retrospectively extracted from patient files and from contact tracing notes. The compliance to the universal testing protocol was high with 90%. Out of 6940 tested patients, 27 new severe acute respiratory syndrome coronavirus‐2 infections (0.4%) were detected. Seven of those COVID‐19 cases (26% of all new cases) were asymptomatic and had no positive contact history, but were identified through a positive PCR test. The number needed to identify an asymptomatic patient was 425 in the first wave of the epidemic, 1218 in the low incidence phase. The specificity of the method was above 99.9%. Universal PCR testing was highly accepted by staff as demonstrated by high compliance. The costs to detect one asymptomatic case in future studies need to be traded off against the costs and damage caused by potential outbreaks of COVID‐19.
Like human Th1 cells, mouse Th1 cells also secrete IFN‐γ upon stimulation with a superagonistic anti‐CD28 monoclonal antibody (CD28‐SA). Crosslinking of the CD28‐SA via FcR and CD40‐CD40L interactions greatly increased IFN‐γ release. Our data stress the utility of the mouse as a model organism for immune responses in humans.
Fibroblast growth factors (FGFs) and their receptors (FGFRs) are involved in demyelinating pathologies including multiple sclerosis (MS). In our recent study, oligodendrocyte‐specific deletion of FGFR1 resulted in a milder disease course, less inflammation, reduced myelin and axon damage in EAE. The objective of this study was to elucidate the role of oligodendroglial FGFR2 in MOG\(_{35-55}\)‐induced EAE. Oligodendrocyte‐specific knockout of FGFR2 (Fgfr2\(^{ind-/-}\)) was achieved by application of tamoxifen; EAE was induced using the MOG\(_{35-55}\) peptide. EAE symptoms were monitored over 62 days. Spinal cord tissue was analysed by histology, immunohistochemistry and western blot. Fgfr2\(^{ind-/-}\) mice revealed a milder disease course, less myelin damage and enhanced axonal density. The number of oligodendrocytes was not affected in demyelinated areas. However, protein expression of FGFR2, FGF2 and FGF9 was downregulated in Fgfr2\(^{ind-/-}\) mice. FGF/FGFR dependent signalling proteins were differentially regulated; pAkt was upregulated and pERK was downregulated in Fgfr2\(^{ind-/-}\) mice. The number of CD3(+) T cells, Mac3(+) cells and B220(+) B cells was less in demyelinated lesions of Fgfr2\(^{ind-/-}\) mice. Furthermore, expression of IL‐1β, TNF‐α and CD200 was less in Fgfr2\(^{ind-/-}\) mice than controls. Fgfr2ind−/− mice showed an upregulation of PLP and downregulation of the remyelination inhibitors SEMA3A and TGF‐β expression. These data suggest that cell‐specific deletion of FGFR2 in oligodendrocytes has anti‐inflammatory and neuroprotective effects accompanied by changes in FGF/FGFR dependent signalling, inflammatory cytokines and expression of remyelination inhibitors. Thus, FGFRs in oligodendrocytes may represent potential targets for the treatment of inflammatory and demyelinating diseases including MS.
Although the bone marrow contains most hematopoietic activity during adulthood, hematopoietic stem and progenitor cells can be recovered from various extramedullary sites. Cells with hematopoietic progenitor properties have even been reported in the adult brain under steady‐state conditions, but their nature and localization remain insufficiently defined. Here, we describe a heterogeneous population of myeloid progenitors in the leptomeninges of adult C57BL/6 mice. This cell pool included common myeloid, granulocyte/macrophage, and megakaryocyte/erythrocyte progenitors. Accordingly, it gave rise to all major myelo‐erythroid lineages in clonogenic culture assays. Brain‐associated progenitors persisted after tissue perfusion and were partially inaccessible to intravenous antibodies, suggesting their localization behind continuous blood vessel endothelium such as the blood‐arachnoid barrier. Flt3\(^{Cre}\) lineage tracing and bone marrow transplantation showed that the precursors were derived from adult hematopoietic stem cells and were most likely continuously replaced via cell trafficking. Importantly, their occurrence was tied to the immunologic state of the central nervous system (CNS) and was diminished in the context of neuroinflammation and ischemic stroke. Our findings confirm the presence of myeloid progenitors at the meningeal border of the brain and lay the foundation to unravel their possible functions in CNS surveillance and local immune cell production.
The liver‐derived, circulating transport protein transthyretin (TTR) is the cause of systemic hereditary (ATTRv) and wild‐type (ATTRwt) amyloidosis. TTR stabilization and knockdown are approved therapies to mitigate the otherwise lethal disease course. To date, the variety in phenotypic penetrance is not fully understood. This systematic review summarizes the current literature on TTR pathophysiology with its therapeutic implications. Tetramer dissociation is the rate‐limiting step of amyloidogenesis. Besides destabilizing TTR mutations, other genetic (RBP4, APCS, AR, ATX2, C1q, C3) and external (extracellular matrix, Schwann cell interaction) factors influence the type of onset and organ tropism. The approved small molecule tafamidis stabilizes the tetramer and significantly decelerates the clinical course. By sequence‐specific mRNA knockdown, the approved small interfering RNA (siRNA) patisiran and antisense oligonucleotide (ASO) inotersen both significantly reduce plasma TTR levels and improve neuropathy and quality of life compared to placebo. With enhanced hepatic targeting capabilities, GalNac‐conjugated siRNA and ASOs have recently entered phase III clinical trials. Bivalent TTR stabilizers occupy both binding groves in vitro, but have not been tested in trials so far. Tolcapone is another stabilizer with the potential to cross the blood–brain barrier, but its half‐life is short and liver failure a potential side effect. Amyloid‐directed antibodies and substances like doxycycline aim at reducing the amyloid load, however, none of the yet developed antibodies has successfully passed clinical trials. ATTR‐amyloidosis has become a model disease for pathophysiology‐based treatment. Further understanding of disease mechanisms will help to overcome the remaining limitations, including application burden, side effects, and blood–brain barrier permeability.
Es wurde die diagnostische Genauigkeit fünf verschiedener Rekonstruktionsalgorithmen der DE-CTA der Becken-Bein-Gefäße von Patienten mit pAVK an einem Dual-Source CT mit dem diagnostischen Goldstandard der DSA verglichen. Die DE-CTA zeigte eine exzellente Sensitivität an Becken- und Beingefäßen hinsichtlich einer korrekten Stenosegraduierung, unabhängig von der Rekonstruktion. Die Standard MPR zeigte insgesamt die besten Ergebnisse und ist deshalb vor allem in nicht eindeutigen Fällen die Methode der Wahl. Die Auswertung von DE-CS MPRs, DE-CS MIPs, gekrümmten MPRs oder die semi-automatische quantitative Stenosegradauswertung bringt keinen Zugewinn an diagnostischer Performance im Vergleich zur visuellen Auswertung der Standard MPR. Das Ausmaß der Gefäßwandverkalkungen und die intraluminale Kontrastierung hatten keinen signifikanten Effekt auf die Sensitivität zur Erkennung signifikanter Stenosen an den Becken- und Beingefäßen. Jedoch verschlechterten sich die Spezifität und die Genauigkeit der Rekonstruktionen, welche auf der DE-Knochen- und DE-Plaqueentfernung basieren, signifikant in stark verkalkten oder nur schwach kontrastierten Gefäßsegmenten. Beim Auswerten von DE-CS Rekonstruktionen ist infolgedessen vor allem in schlecht kontrastierten oder stark verkalkten Gefäßen eine weitere Verifizierung des Befundes nötig, um unnötige invasive DSAs als Folge von falsch positiven CTA-Befunden zu reduzieren.
Die vorliegende Arbeit untersucht den Natriumgehalt verschiedener Kompartimente des Körpers mittels Magnetresonanztomographie (= MRT).
Die Korrelation zwischen erhöhtem Salzkonsum und arterieller Hypertonie ist bereits umfangreich analysiert worden. Für das Verständnis der pathophysiologischen Zustände und deren Regulation, ist eine Quantifizierung von Natriumkonzentrationen in verschiedenen Gewebearten bedeutsam. Die exakte Messung von Natriumkonzentrationen im menschlichen Gewebe ist derzeit experimentell. Im Rahmen der hier vorgelegten Arbeit wurden die Natriumkonzentrationen von Haut und Skelettmuskel mittels 23Na Magnetresonanztomographie (= 23 Na MRT) im menschlichen Körper quantifiziert.
Natriummessungen wurden bei Patienten mit primärem Hyperaldosteronismus (= PHA), bei Patienten mit essentieller Hypertonie (= EH), sowie einer gesunden Kontrollgruppe vorgenommen.
Die Ergebnisse zeigten, dass Haut und Skelettmuskel Speicherorgane für Natrium im menschlichen Körper darstellen. Durch gezielte Therapie waren die Natriumkonzentrationen in beiden Speicherorganen modulierbar
In der vorliegenden Studie wurde der Zusammenhang des depressiven Syndroms mit dem Vitamin D-Spiegel an einer Stichprobe gerontopsychiatrischer Patienten (n = 140) der Neurogerontopsychiatrischen Tagesklinik Würzburg untersucht. Die Depressivität der Patienten zu Beginn und im Verlauf der Behandlung wurde zum einen mittels der ICD-10-Klassifikation, zum anderen mittels des Scores auf der GDS- und Hamilton-Skala zu Beginn und Ende des Aufenthalts in der Tagesklinik sowie bei einer poststationären Kontrolle bestimmt. Der Vitamin D-Spiegel wurde bei Behandlungsbeginn bestimmt und im Falle eines Mangels 1000 IU Vitamin D am Tag oral substituiert. Hierbei zeigte sich kein Zusammenhang zwischen der Ausprägung des depressiven Syndroms und dem Vitamin D-Spiegel zu Beginn der Behandlung. Dagegen stellte sich heraus, dass Patienten mit einem höheren Spiegel eine deutlichere Verbesserung der depressiven Symptome auf der GDS im Verlauf der Behandlung erfuhren. Außerdem bestand eine signifikante negative Korrelation zwischen BMI und Vitamin D-Spiegel sowie eine Abhängigkeit der Spiegelhöhe von der Jahreszeit. Vitamin D könnte nach den Ergebnissen dieser Studie möglicherweise eine wirkungssteigernde und nebenwirkungsarme Komedikation in der antidepressiven Therapie von älteren psychisch erkrankten Menschen darstellen. Es bedarf weiterer ausführlicher Forschung über den neurophysiologischen Zusammenhang zwischen Vitamin D und der Schwere einer depressiven Erkrankung. Besonders hinsichtlich der Verwendung von Vitamin D als Komedikation gilt es, weitere intensive Forschung in Form von gut designten, randomisierten Fall-Kontroll-Studien und prospektiven Interventionsstudien zu betreiben, um die Therapie von depressiven Patienten im höheren Lebensalter weiter zu verbessern.
Das Mikromilieu solider Tumor (tumor mircoenvironment, TME) weist verschiedene Besonderheiten auf, von denen bekannt ist, dass sie zu Chemotherapieresistenz und Tumorprogression beitragen können. Neben der Extrazellulären Matrix (ECM), den cancer associated cells (CAC) und diversen Entzündungszellen tragen auch chemische und physikalische Besonderheiten (Hypoxie, Azidose, erhöhter Gewebedruck, oxidativer Stress und Nährstoffmangel) zu Tumorprogression und Chemotherapieresistenz bei. Zudem wissen wir, dass Hitzeschock-Proteine (HSPs), Toll-like Rezeptoren (TLRs) und ABC-Transporter mit erhöhter Chemotherapieresistenz und Tumorprogression im Pankreas- und Kolonkarzinom einhergehen.
Hier wurde untersucht, ob ein in vitro induzierter Nährstoffmangel im HT29 Kolonkarzinom, im Panc-1 Pankreaskarzinom und im MIA PaCa-2 Pankreaskarzinom zu einer gesteigerten Expression von HSP70, HSP90, MDR1, ABCB5 und TLR1 bis TLR10 auf mRNA und Proteinebene führt. Zudem wurde unter allen Versuchsbedingungen die Stoffwechselaktivität über einen MTS-Test gemessen. Der Nährstoffmangel wurde über die Kultivierung in einem Hybridomamedium, welches als proteinfreies Medium gilt und über die Kultivierung in einem serumfreien Medium induziert.
Es zeigte sich, dass insbesondere die entdifferenzierte Panc-1 Pankreaskarzinomzelllinie eine erhöhte Resistenz gegenüber dem induzierten Nährstoffmangel aufwies. Auf mRNA-Ebene zeigten sich bei allen drei Tumorzelllinien deutliche Expressionssteigerungen. Diese waren insbesondere im Hybridomamedium nachweisbar und traten beim HT29-Kolonkarzinom nach 48h und im Panc-1 Pankreaskarzinom bereits nach 24h auf. Besonders intensive Expressionssteigerungen konnten im HT29 Kolonkarzinom bei ABCB5, TLR7 und TLR9 nachgewiesen werden. Die Expression von MDR1 war insbesondere im MIA PaCa-2 Pankreaskarzinom gesteigert. Auf Proteinebene konnte im HT29 Kolonkarzinom eine Expressionssteigerung bei HSP90 und TLR6 nachgewiesen werden.
Die Ergebnisse lassen zwei Interpretationen zu. Zum einen könnte über den Nährstoffmangel eine aggressivere Subpopulation selektioniert worden sein. In diesem Zusammenhang konnten die Expressionsdaten des Tumorstammzellmarkers CD133 leider nicht ausgewertet werden. Alternativ kann angenommen werden, dass die untersuchten Tumorzelllinien ihren aggressiven Phänotyp erst unter Nährstoffmangelbedingungen, wie wir sie regelmäßig in soliden Tumoren finden, zur Expression bringen.
Die Nikotinamid N-Methyltransferase (NNMT) wurde als wichtiger Regulator des Energiemetabolismus in Fettzellen beschrieben. So bewahrt ein NNMT Knock-down Mäuse vor einer nahrungsinduzierten Adipositas und bei reduzierter NNMT-Expression in weißen 3T3-L1 Adipozyten zeigen diese einen erhöhten zellulären Sauerstoffverbrauch.
Für den Adipozytenstoffwechsel ist die insulinstimulierte Glukoseaufnahme wesentlich. Um den Einfluss eines NNMT-Knock-downs auf diese zu untersuchen wurde unter Nutzung der Substratspezifitäten des prokaryotischen und eukaryotischen Isoenzyms der Glukose-6-phosphat-Dehydrogenase ein enzymatisch-photometrischer Assay zur Messung der Glukoseaufnahmerate in adhärenten weißen 3T3-L1 und braunen Adipozytenkulturen entwickelt.
Mit lentiviraler Transduktion wurde in den Adipozytenkulturen ein persistenter NNMT-Knock-down induziert. Die NNMT-Aktivität wurde mit einem fluoreszenzbasierten Assay gemessen und die Glukoseaufnahmerate in deren Abhängigkeit bestimmt.
Die Reduktion der NNMT-Aktivität verminderte die Glukoseaufnahmerate der 3T3-L1 Adipozyten sowohl basal, wie auch unter Insulinstimulation. Braune Adipozyten hingegen zeigten bei verringerter NNMT-Aktivität eine erhöhte insulinstimulierte Glukoseaufnahmerate, aber keinen Unterschied der basalen Glukoseaufnahmerate.
Dieser differenzielle Einfluss auf die Glukoseaufnahme weißer und brauner Adipozyten stärkt die wichtige Rolle der NNMT, die ihr zur Regulation des Fettzellstoffwechsels zugeschrieben wird und enthüllt erstmals eine direkte Wirkung auf braune Adipozyten.
Die alveoläre Echinokokkose (AE), verursacht durch das Metacestoden- Larvenstadium des Fuchsbandwurms Echinococcus multilocularis (E. multilocularis), ist eine lebensbedrohliche Zoonose der nördlichen Hemisphäre mit eingeschränkten therapeutischen Möglichkeiten. Bei der Suche nach neuen Therapeutika haben Mitogen-activated Proteinkinase (MAPK) -Kaskaden als pharmakologische Zielstrukturen aufgrund ihrer essentiellen Rolle bei der Zellproliferation und -differenzierung ein großes Potenzial. In der vorliegenden Arbeit wurden durch BLAST- und reziproke BLAST-Analysen elf potenzielle MAPK Kinase Kinasen (MAP3K), fünf potenzielle MAPK Kinasen (MAP2K) und sechs potenzielle MAPK im E. multilocularis-Genom identifiziert, die teils hoch konserviert sind und in nahezu allen Entwicklungsstadien des Parasiten exprimiert werden. Diese Erkenntnisse lassen auf ein komplexes MAPK-Signaltransduktions- system in E. multilocularis mit großer Bedeutung für den Parasiten schließen. Transkriptomdatenanalysen und Whole Mount in Situ Hybridisierung (WMISH) zeigten, dass emmkkk1 (EmuJ_000389600) als einzige MAP3K neben der Expression in postmitotischen Zellen in besonderem Maße in proliferativen Stammzellen des Parasiten exprimiert wird und somit eine wichtige Rolle bei der Differenzierung von Stammzellen spielen könnte. In Yeast-Two-Hybrid (Y2H) -Wechselwirkungsassays wurden Interaktionen von mehreren upstream- (EmGRB2) und downstream- wirkenden Signalkaskadekomponenten des JNK (EmMKK3, EmMPK3) und ERK (EmMKK3, EmMPK4) -Signalwegs gefunden. Daraus lässt sich schließen, dass EmMKKK1, analog zu seinem humanen Homolog HsM3K1, eine zentrale Rolle bei der Echinococcus-Wachstumsregulation durch Rezeptortyrosinkinasen und vielfältige weitere Funktionen im Parasiten besitzt. Anhand von Erkenntnissen an Platyhelminthes kann daher von einem großen Potenzial dieser neu charakterisierten Signalwege als chemotherapeutische Angriffspunkte ausgegangen werden, wenngleich erste RNA-Interferenz (RNAi)- und Inhibitorstudien an emmkkk1, emmpk1 und emmpk4 keine durchschlagenden Effekte auf das Überleben von Primärzellkulturen und die Bildung von Metacestodenvesikeln zeigten. Zusammenfassend konnte in der vorliegenden Arbeit mit EmMKKK1 und neuen ERK- und JNK-Signalwegen zentrale Komponenten der komplexen MAPK-Signalkaskaden in E. multilocularis identifiziert werden, die höchstwahrscheinlich einen großen Beitrag zur enormen Regenerationsfähigkeit der Echinococcus-Stammzellen leisten und vom Wirt abgeleitete Signale wie Insulin, Epidermaler Wachstumsfaktor (EGF) und Fibroblasten-Wachstumsfaktor (FGF) über EmGRB2 in Proliferationsnetzwerke des Parasiten integrieren. Arzneimittel-Screening-Assays, die auf diese Signalwege abzielen, könnten daher zu alternativen Arzneimitteln führen, die alleine oder in Kombination mit einer bestehenden Chemotherapie (Benzimidazol) die Prognose von für AE-Patienten verbessern könnten.
