Objectives
Trauma evaluation of extremities can be challenging in conventional radiography. A multi-use x-ray system with cone-beam CT (CBCT) option facilitates ancillary 3-D imaging without repositioning. We assessed the clinical value of CBCT scans by analyzing the influence of additional findings on therapy.
Methods
Ninety-two patients underwent radiography and subsequent CBCT imaging with the twin robotic scanner (76 wrist/hand/finger and 16 ankle/foot/toe trauma scans). Reports by on-call radiologists before and after CBCT were compared regarding fracture detection, joint affliction, comminuted injuries, and diagnostic confidence. An orthopedic surgeon recommended therapy based on reported findings. Surgical reports (N = 52) and clinical follow-up (N = 85) were used as reference standard.
Results
CBCT detected more fractures (83/64 of 85), joint involvements (69/53 of 71), and multi-fragment situations (68/50 of 70) than radiography (all p < 0.001). Six fractures suspected in radiographs were ruled out by CBCT. Treatment changes based on additional information from CBCT were recommended in 29 patients (31.5%). While agreement between advised therapy before CBCT and actual treatment was moderate (κ = 0.41 [95% confidence interval 0.35–0.47]; p < 0.001), agreement after CBCT was almost perfect (κ = 0.88 [0.83–0.93]; p < 0.001). Diagnostic confidence increased considerably for CBCT studies (p < 0.001). Median effective dose for CBCT was 4.3 μSv [3.3–5.3 μSv] compared to 0.2 μSv [0.1–0.2 μSv] for radiography.
Conclusions
CBCT provides advantages for the evaluation of acute small bone and joint trauma by detecting and excluding extremity fractures and fracture-related findings more reliably than radiographs. Additional findings induced therapy change in one third of patients, suggesting substantial clinical impact.
Purpose
We used exact matching for a highly balanced comparison of ab interno trabeculectomy (AIT) with the trabectome to trabeculectomy with mitomycin C (TRAB).
Methods
A total of 5485 patients who underwent AIT were exact-matched to 196 TRAB patients by baseline intraocular pressure (IOP), number of glaucoma medications, and glaucoma type. Nearest-neighbor–matching was applied to age. Success was defined as a final IOP of less than 21 mmHg, IOP reduction of at least 20% reduction from baseline, and no secondary surgical interventions. Outcomes were measured at 1, 3, 6, 12, 18, and 24 months.
Results
A total of 165 AIT could be matched to 165 TRAB. The mean baseline IOP was 22.3 ± 5.6 mmHg, and the baseline number of glaucoma medications was 2.7 ± 1.1 in both groups. At 24 months, IOP was reduced to 15.8 ± 5.2 mmHg in AIT and 12.4 ± 4.7 mmHg in TRAB. IOP was lower than baseline at all visits (p < 0.01) and lower in TRAB than AIT (p < 0.01). Glaucoma medications were reduced to 2.1 ± 1.3 in AIT and 0.2 ± 0.8 in TRAB. Compared to baseline, patients used fewer drops postoperatively (p < 0.01) and more infrequently in TRAB than in AIT (p > 0.01). Secondary surgical interventions had the highest impact on success and became necessary in 15 AIT and 59 TRAB patients. Thirty-two challenging events occurred in TRAB and none in AIT.
Conclusion
Both AIT and TRAB reduced IOP and medications. This reduction was more significant in TRAB but at the expense of four times as many secondary interventions.
Purpose
Local treatment of small well-differentiated rectal neuroendocrine tumors (NETs) is recommended by current guidelines. However, although several endoscopic methods have been established, the highest R0 rate is achieved by transanal endoscopic microsurgery (TEM). Since a recently published study about endoscopic full thickness resection (eFTR) showed a R0 resection rate of 100%, the aim of this study was to evaluate both methods (eFTR vs. TEM).
Methods
We retrospectively analyzed all patients with rectal NET treated either by TEM (1999–2018) or eFTR (2016–2019) in two tertiary centers (University Hospital Wuerzburg and Ulm). We analyzed clinical, procedural, and histopathological outcomes in both groups.
Results
Twenty-eight patients with rectal NET received local treatment (TEM: 13; eFTR: 15). Most tumors were at stage T1a and grade G1 or G2 (in the TEM group two G3 NETs were staged T2 after neoadjuvant chemotherapy). In both groups, similar outcomes for en bloc resection rate, R0 resection rate, tumor size, or specimen size were found. No procedural adverse events were noted. Mean procedure time in the TEM group was 48.9 min and 19.2 min in the eFTR group.
Conclusion
eFTR is a convincing method for local treatment of small rectal NETs combining high safety and efficacy with short interventional time.
In peripheral nervous system (PNS), the blood-nerve barrier (BNB) and myelin barrier (MB) are important physiological fences for maintaining the environment for axons, Schwann cells and other associated cells within peripheral nerves. The perineurium surrounding the nerves and endoneurial vessels nourishing the nerves compose the BNB. Schwann cells wrapping around neurons form the MB. Destruction or malfunction of the barriers has been postulated as an initial step in the development of pathologic conditions concerning human peripheral nerves, such as traumatic neuropathy and the disease of chronic inflammatory demyelination polyneuropathy (CIDP).
Tight junction proteins (TJPs) are intercellular junctions building the microstructure of barriers. They play a key role in tightly connecting adjacent cells, controlling the passage of ions, water and other molecules via the paracellular pathway, and maintaining the cell polarity. Among the family of TJPs, claudins are the major structural components which form the backbone of TJs. Certain key TJPs [e.g. claudins (claudin-1, -5, -19, occludin, zona occludens (ZO-1)] have been identified in neural barriers and explored for therapeutic targets. The expression of Cldn12 gene has been documented in human/rodent tibial nerves, spinal cord and DRG. However, the role of claudin-12 in PNS is unknown.
In the present study, we firstly found a loss of claudin-12 immunoreactivity (IR) in male or postmenopausal female patients with painful CIDP or non-inflammatory polyneuropathy (PNP). Then, we utilized male and female Cldn12-KO mice and the chronic constriction injury (CCI) model. Cldn12 mRNA and IR were reduced in WT mice after nerve injury. Deletion of Cldn12 via general knockout (KO) induced mechanical allodynia at baseline level and after CCI in time-dependent manner in male mice. KO of Cldn12 in males resulted in loss of small axons, perineurial barrier and MB breakdown, as well as TJP complex disruption with claudin-1, -19 and Pmp22 reduction. Moreover, local Cldn12 siRNA application mimicked mechanical allodynia and MB breakdown.
In conclusion, claudin-12 deficiency is associated with painful CIDP/non-inflammatory PNP. Claudin-12 is a regulatory TJP crucial for mechanical nociception, perineurial barrier and MB integrity, and proper TJP composition in mice. Therefore, further investigating the functions of claudin-12 and its mechanism is important to prompt the development of new therapeutic approaches for painful neuropathies.
Objectives
The present investigation aimed to evaluate the subjective perception of deformational cranial asymmetries by different observer groups and to compare these subjective perceptions with objective parameters.
Materials and methods
The 3D datasets of ten infants with different severities of deformational plagiocephaly (DP) were presented to 203 observers, who had been subdivided into five different groups (specialists, pediatricians, medical doctors (not pediatricians), parents of infants with DP, and laypersons). The observers rated their subjective perception of the infants’ cranial asymmetries using a 4-point Likert-type scale. The ratings from the observer groups were compared with one another using a multilevel modelling linear regression analysis and were correlated with four commonly used parameters to objectively quantify the cranial asymmetries.
Results
No significant differences were found between the ratings of the specialists and those of the parents of infants with DP, but both groups provided significantly more asymmetric ratings than did pediatricians, medical doctors, or laypersons. Moreover, the subjective perception of cranial asymmetries correlated significantly with commonly used parameters for objectively quantifying cranial asymmetries.
Conclusions
Our results demonstrate that different observer groups perceive the severity of cranial asymmetries differently. Pediatricians’ more moderate perception of cranial asymmetries may reduce the likelihood of parents to seek therapeutic interventions for their infants. Moreover, we identified some objective symmetry-related parameters that correlated strongly with the observers’ subjective perceptions.
Clinical relevance
Knowledge about these findings is important for clinicians when educating parents of infants with DP about the deformity.
Das Ziel dieser Studie war es, die Pathomechanismen der Enzepahlopathie, welche während einer akuten Nierenschädigung beobachtet werden kann, zu untersuchen. Wir konzipierten ein Zellkulturmodell, welches aus der cerebralen Endothelzelllinie cEND und der Nierentubuluszelllinie HK-2 bestant. Die Nierenzellen wurden einem Sauerstoff-Glukose-Entzug zugeführt, um eine akute Nierenschädigung zu simulieren. Nach weiterer Behandlung mit zwei urmämischen Toxinen (Indoxylsulfate und Indolessigsäure) und gemeinsamer Inkubation beider Zelllinien für insgesamt 48 Stunden wurden die cEND-Zellen geerntet und die Expression von Tight junction-Proteinen und Glukosetransportern untersucht.
Cell death is an essential aspect of life that plays an important role for successful development and tissue remodeling as well as for diseases. There are several different types of cell death that differ from each other in morphological, functional and biochemical ways. Regulated cell death that occurs in physiological processes is generally equated with programmed cell death (PCD), whereby apoptosis is the most studied form of PCD. Ferroptosis is a form of regulated cell death and unique in its requirements for iron and lipid peroxidation. It is linked to numerous biological processes, such as amino acid metabolism, phospholipid metabolism and sterol synthesis. Cholesterol biosynthesis is a complex pathway with a large number of enzymes and substrates that are potential target points for cellular dysfunctions. Motivated by the results from a CRISPR-based genetic screening in this thesis, we focused on 7-dehydrocholesterol reductase (DHCR7), the enzyme responsible for conversion of 7-dehydrocholesterol (7-DHC) to cholesterol. In this work we focused on the ferroptosis sensitive cell line HT1080 and generated a series of models to address the importance of DHCR7 in ferroptosis. Using CRISPR/Cas9, HT1080 DHCR7_KO and DHCR7/SC5D_KO cell lines were generated and used to validate their sensitivity against ferroptosis inducers and sterol consumption. We could show that 7-DHC is a strong antiferroptotic agent that could prevent cell death in genetic models as well as when supplemented directly to cells. Importantly, all the results obtained were subsequently confirmed in isogenic reconstituted pairs from the HT1080 DHCR7/SC5D_KO. Moreover, we demonstrate that this protective effect is not due to an inherent and unspecific resistance as the sensitivity to non-ferroptotic stimuli was equally effective in killing the HT1080 DHCR7_KO and DHCR7/SC5D_KO cell lines. We could also show that selenium present in the media has a strong impact on the activity of 7-DHC and this is because in its absence the effective concentration is rapidly decreased. Surprisingly we also demonstrate that removing sterol from cell culture triggers ferroptosis in cells unable to synthesize 7-DHC, suggestive that this could be used as a novel mechanism to trigger ferroptosis. Ultimately, in the present work we could show that unlike previously reported, 7-DHC is not only a toxic intermediate of the cholesterol biosynthesis pathway but under specific circumstances it has a strong pro-survival effect.
Eine Prognoseeinschätzung bei Patienten mit hypoxischer Enzephalopathie (HIE) nach Reanimation wird frühestens 72 Stunden nach Reanimation empfohlen. Bis zu diesem Zeitpunkt besteht eine für Ärzte und Angehörige belastende prognostische Lücke. Und auch nach 72 Stunden bestehen nur ungenaue Angaben zum weiteren Vorgehen, sodass eine fundierte Prognoseeinschätzung aktuell eine deutliche Herausforderung im Alltag klinisch tätiger Ärzte darstellt.
Der Nervus opticus ist als Bestandteil des zentralen Nervensystems mit dem Liquorsystem verbunden. Intrazerebrale Druckerhöhungen wirken sich daher unmittelbar auf die ihn ummantelnde Nervenscheide und deren Durchmesser aus, sodass sich die Bestimmung des Optikusnervenscheidendurchmessers (ONSD) mittels transorbitaler Sonographie in der Diagnostik unterschiedlicher intrakranieller Erkrankungen bereits bewährt hat. Das Krankheitsbild der HIE wurde als weiteres mögliches Einsatzgebiet des ONSD jedoch bisher nicht untersucht.
Ziel dieser Dissertation war es daher, den ONSD grundsätzlich auf seine Verlässlichkeit als Prognoseparameter bei HIE nach Reanimation zu überprüfen. Besonderes Augenmerk lag hierbei auf der Ermöglichung einer frühzeitigen Prognoseeinschätzung innerhalb von 24 Stunden sowie auf der Definition eines prognostischen Cut-Off-Wertes als klare Entscheidungshilfe für weitere therapeutische Strategien.
24, 48 und 72 Stunden nach Reanimation werden signifikant unterschiedliche ONSD unter überlebenden und verstorbenen Patienten nachgewiesen. Letztere weisen dabei im Vergleich sowohl höhere als auch im zeitlichen Verlauf signifikant ansteigende ONSD-Werte auf. Als prognostischer Cut-Off-Wert konnte eine Grenze bei 5,75mm festgelegt werden.
Zusammenfassend stellt die sonographische Bestimmung des ONSD eine sinnvolle Zusatzdiagnostik in der Prognoseeinschätzung bei Patienten mit HIE nach Reanimation dar.
Brustkrebs ist die häufigste maligne Erkrankung der Frau. Die Therapie setzt sich in der Regel individuell aus den Bausteinen der chirurgischen Tumorexzision, der Bestrahlung und der systemischen Therapie zusammen. Daneben gewinnt die ketogene Diät als supportiver Therapieansatz immer mehr an Aufmerksamkeit und Forschungsinteresse. Diese Ernährungsform imitiert durch starke Restriktion der Kohlenhydratzufuhr den Fastenstoffwechsel, da Blutzucker- und konsekutiv auch Insulinspitzen im Blut vermieden werden. Eine tragende Rolle kommt dabei der Bildung von Ketonkörpern, allen voran Betahydroxybutyrat, zu, die sowohl in den Tumorstoffwechsel als auch in immunologische Prozesse eingreifen können. In dieser Arbeit wurde ausgewählten Brustkrebszellen 3 mM Betahydroxybutyrat zugesetzt und ihr Wachstumsverhalten, ihre Chemo- und Radiosensitivität im Vergleich zu Kontrollzellen erfasst. Die Kontrollzellen wurden identisch behandelt, jedoch wurde Ihnen kein Betahydroxybutyrat zugefügt. Es zeigte sich dabei kein statistisch signifikanter Unterschied zwischen den beiden Zellgruppen.
Ziel der Studie war es, das postoperative Blutungsrisiko von Patienten im Rahmen einer elektiven Operation zu untersuchen die aufgrund einer oralen Antikoagulation die überbrückende Therapie (Bridging) mit niedermolekularem Heparin erhielten.
Eine monozentrische, retrospektive Fall-Kontrollstudie (n = 526 Patienten) wurde durchgeführt, um die Inzidenz schwerer und leichter postoperativer Blutungen sowie thromboembolischer Ereignisse, die Verweildauer und die Mortalität im Krankenhaus zu ermitteln. In den Ergebnissen konnte eine erhöhte postoperative Inzidenz von schweren (8% vs. 1%; p <0,001) und leichten (14% vs. 5%; p <0,001) Blutungsereignissen festgestellt werden. Thromboembolische Ereignisse waren in beiden Gruppen selten (1% vs. 2%; p = 0,45). Es kam zu keiner erhöhten Mortalität (1,5% vs. 1,9%). Als unabhängige Risikofaktoren für schwere postoperative Blutungen konnten die volle therapeutische Dosis von LMWH, Niereninsuffizienz und das eingriffsspezifische Blutungsrisiko bestimmt werden. Unter Berücksichtigung der Ergebnisse dieser Studie sollte die routinemäßige Bridgingtherapie in der allgemeinen und viszeralen Chirurgie sorgfältig abgewogen werden.
The adaptive immune system is known to provide highly specific and effective immunity against a broad variety of pathogens due to different effector cells. The most prominent are CD4+ T-cells which differentiate after activation into distinct subsets of effector and memory cells, amongst others T helper 1 (Th1) cells. We have recently shown that mouse as well as human Th1 cells depend on T cell receptor (TCR) signals concomitant with CD28 costimulation in order to secrete interferon (IFN) which is considered as their main effector function. Moreover, there is a class of anti-CD28 monoclonal antibodies that is able to induce T cell (re-)activation without concomitant TCR ligation. These so-called CD28-superagonists (CD28-SA) have been shown to preferentially activate and expand CD4+ Foxp3+ regulatory T (Treg) cells and thereby efficaciously conferring protection e.g. against autoimmune responses in rodents and non-human primates. Considering this beneficial effect, CD28-SA were thought to be of great impact for immunotherapeutic approaches and a humanized CD28-SA was subjected to clinical testing starting with a first-in-man trial in London in 2006. Unexpectedly, the volunteers experienced life-threatening side effects due to a cytokine release syndrome (CRS) that was unpredicted by the preclinical studies prior to the trial. Retrospectively, CD4+ memory T cells within the tissues were identified as source of pro-inflammatory cytokines released upon CD28-SA administration. This was not predicted by the preclinical testing indicating a need for more reliable and predictive animal models. Whether mouse CD4+ T cells are generally irresponsive to CD28-SA stimulation or rather the lack of a bona fide memory T cell compartment in cleanly housed specific-pathogen-free (SPF) mice is the reason why the rodent models failed to predict the risk for a CRS remained unclear. To provide SPF mice with a true pool of memory/effector T cells, we transferred in vitro differentiated TCR-transgenic OT-II Th1 cells into untreated recipient mice. Given that Treg cells suppress T cell activation after CD28- SA injection in vivo, recipients were either Treg-competent or Treg-deficient, wild type or DEREG mice, respectively. Subsequent CD28-SA administration resulted in induction of systemic pro-inflammatory cytokine release, dominated by IFN, that was observed to be much more pronounced and robust in Treg-deficient recipients. Employing a newly established in vitro system mirroring the in vivo responses to CD28-SA stimulation of Th1 cells revealed that antigen-presenting cells (APCs) amplify CD28-SAinduced IFN release by Th1 cells due to CD40/CD40L-interactions. Thus, these data are the first to show that mouse Th1 cells are indeed sensitive to CD28-SA stimulation in vivo and in vitro responding with strong IFN release accompanied by secretion of further pro-inflammatory cytokines, which is compatible with a CRS. In conclusion, this study will facilitate preclinical testing of immunomodulatory agents providing a mouse model constituting more “human-like” conditions allowing a higher degree of reliability and translationability.
Die Rheumatoide Arthritis ist eine häufig auftretende, chronisch entzündliche Systemerkrankung und wird bei bis zu einem Drittel der Patienten mit einer T-LGL-Leukämie diagnostiziert. Wie häufig klonale T-LGL-Zellen bei Patienten mit Rheumatoider Arthritis auftreten, ist ungeklärt.
Ziel dieser Studie war es, die Oberflächenantigene der T-Lymphozyten in einem Patientenkollektiv mit Rheumatoider Arthritis zu bestimmen. Der Fokus lag dabei auf der Prävalenz von klonalen T-LGL-Zellexpansionen und möglichen Risikofaktoren.
Hierfür wurden zwischen November 2013 und August 2015 527 Patienten mit Rheumatoider Arthritis mittels Durchflusszytometrie untersucht. Zur Bestätigung der Klonalität erfolgte bei Patienten mit auffälligem Immunphänotyp eine PCR (Polymerase-Kettenreaktion)-Analyse.
Bei 19 Patienten konnte eine klonale T-LGL-Zellexpansion festgestellt werden, was einer Prävalenz von 3,6% entspricht. Das Auftreten von klonalen T-LGL-Zellen war mit einer TNFα-Inhibitoren-Therapie (p=0,01) und deren Dauer assoziiert (p=0,01).
Ob die klonalen T-LGL-Zellen Ausdruck der Autoimmunerkrankung oder Vorläuferzellen einer T-LGL-Leukämie sind, bleibt offen. Die Patienten werden mit einer klonalen T-LGL-Zellexpansion unklarer Signifikanz beschrieben.
Die akute Appendizitis stellt die häufigste Ursache des akuten Abdomens jenseits des Säuglings- und Kleinkindalters dar. Gerade aufgrund der erschwerten Diagnostik und des häufig foudroyanten Verlaufes bei jüngeren Kindern sind eine frühzeitige Diagnosestellung und adäquate Therapie dieser Erkrankung von unermesslichem Wert.
Diese retrospektive Studie befasst sich mit Kindern und Jugendlichen unter 18 Jahren, die im Zeitraum vom 01.05.2008 bis 31.05.2013 in der Abteilung für Kinderchirurgie des Universitätsklinikums Würzburg appendektomiert und postoperativ betreut wurden. Simultaneingriffe, wie z.B. Appendektomie bei Malrotation, wurden hierbei ausgeschlossen.
Ziel der vorliegenden Dissertation war es unter anderem, das präoperative Management bezüglich der Diagnostik einer Appendizitis darzustellen und zu beurteilen. Hierbei wurde auch die Bedeutung der Leukozytenzahl sowie des C-reaktiven Proteins untersucht. Ein weiterer Fokus lag darauf, die intraoperative Einschätzung des makroskopischen Befundes durch den Operateur dem jeweiligen histopathologischen Befund gegenüberzustellen. Darüber hinaus wurde das Patientengut hinsichtlich des Verteilungsmusters von Alter und Geschlecht beleuchtet und der Frage nachgegangen, ob es einen möglichen Zusammenhang zwischen dem Auftreten einer Appendizitis und der jeweiligen Jahreszeit bzw. Wetterlage gibt. Abschließend lag ein Schwerpunkt der Arbeit darauf, den Stellenwert der laparoskopischen Appendektomie bei Kindern am UKW zu ermitteln und mögliche Vorteile dieses minimalinvasiven Operationsverfahrens gegenüber der konventionellen Appendektomie zu untersuchen. Die Einordnung und Diskussion der Ergebnisse erfolgte anhand der aktuellen Literatur beziehungsweise Studienlage.
Das Herz ist physiologisch auf einen fein regulierten und ausgeglichenen bioenergetischen Energiehaushalt angewiesen, um auf akute Belastungssituationen adäquat reagieren zu können und oxidativen Stress zu vermeiden. Ca2+ reguliert zentral sowohl die zyklischen Kontraktions-/Relaxationsprozesse (ECC) als auch unmittelbar den mitochondrialen Metabolismus. Der ECC liegt in den Kardiomyozyten die Ca2+- Freisetzung durch die RyR2 zu Grunde; die IP3 Rezeptoren des sarkoplasmatischen Retikulums (SR) führen davon unabhängig zu einer Ca2+ Freisetzung aus dem SR. Diese IP3R vermittelten Signale werden in den räumlich nahe gelegenen Mitochondrien zum Teil über den mRyR1 in die mitochondriale Matrix aufgenommen und stimulieren dort langfristig die oxidative Phosphorylierung und den Erhalt der antioxidativen Kapazität. Die enge räumliche Nähe zwischen SR und Mitochondrien wird durch Strukturproteine wie Mitofusin 2 (Mfn2) ergänzt, die das SR mit der äußeren Mitochondrienmembran koppeln und so die Ca2+-Interaktion beeinflussen. Ziel der Arbeit war, den Effekt von Mfn2 Defizienz auf die IP3 induzierte mitochondriale Ca2+-Regulation in Kardiomyozyten zu evaluieren. Dazu erfolgten Fluoreszenzfärbungen an adulten isolierten Ventrikelkardiomyozyten kardiospezifischer Mfn2 Knock-Out (KO) Mäusen bzw. deren wildtypischen Geschwistertieren (WT). Erhobene Parameter umfassten das mitochondriale Ca2+, das mitochondriale Membranpotenzial, die mitochondriale Superoxidbildung und mitochondriale ATP-Gehalt. Die Ergebnisse bestätigten eine Signalachse, bei der die Stimulation von isolierten murinen Kardiomyozyten mit dem IP3 Agonisten ET-1 zu einer mitochondrialen Ca2+ Aufnahme führte, dem Erhalt des mitochondrialen Membranpotenzials diente und der ATP Gehalt stiegt. Bei induzierter kardiospezifischer Ablation von Mfn2 geht diese SR-mitochondriale Interaktion verloren, und es entstand ein energetisches Defizit sowie eine verminderte Superoxidbildung. Bei beta-adrenerger Stimulation mit Isoproterenol (ISO) resultierte in WT zwar eine mitochondriale Ca2+-Aufnahme, allerdings ein Abfall des ATP-Gehaltes. In den Mfn2 defizienten Kardiomyozyten zeigte sich eine Steigerung des ATP-Gehaltes auch auf beta-adrenerge Stimulation, die einen energetischen Kompensationsmechanismus in den Mfn2 KO Tieren vermuten lässt. Dies identifiziert Mfn2 als kritische Strukturkomponente für die basale bioenergetische Adaptation der durch IP3R-mRyR1 vermittelten Signalachse unter physiologischen Bedingungen.
Hintergrund
Intensiv- und Beatmungskapazitäten sind für die Behandlung COVID-19-erkrankter Patienten essenziell. Unabhängig davon beanspruchen auch schwer verletzte Patienten häufig Intensiv- und Beatmungskapazitäten. Daraus ergibt sich folgende Fragestellung: Führt eine Ausgangsbeschränkung zu einer Reduktion schwer verletzter Patienten, und kann hierdurch mit frei werdenden Intensivkapazitäten gerechnet werden?
Material und Methoden
Es erfolgte eine retrospektive Auswertung schwer verletzter Patienten mit einem Injury Severity Score (ISS) ≥16 zwischen dem 17.03.2020 und 30.04.2020 (landesweiter Shutdown) an einem überregionalen Traumazentrum. Erfasst wurden der Unfallmechanismus, ISS, Versicherungsträger (BG vs. GKV/PKV), ob es sich um einen dokumentierten Suizidversuch handelte, und ob eine operative Intervention innerhalb der ersten 24 h erforderlich war. Als Kontrollgruppe wurden die Daten des gleichen Zeitraums der Jahre 2018 und 2019 ausgewertet.
Ergebnisse
Es konnte keine wesentliche Veränderung bezüglich der Anzahl an schwer verletzten Patienten festgestellt werden (2018 n = 30, 2019 n = 23, 2020 n = 27). Es zeigten sich insgesamt keine deutlichen Veränderungen der absoluten Zahlen bezüglich der Intensivpflichtigkeit in den ersten 24 h und der Beatmungspflichtigkeit beim Verlassen des Schockraums. Die Anzahl an Patienten, die eine Operation innerhalb der ersten 24 h nach Eintreffen im Schockraum benötigten, war 2020 sogar leicht erhöht, jedoch nicht statistisch signifikant. Der durchschnittliche ISS blieb konstant. Bezüglich der Unfallursache zeigte sich 2020 kein Motorradfahrer, der einen nicht berufsgenossenschaftlich versicherten Unfall erlitt (2018 n = 5, 2019 n = 4, 2020 n = 0). Es wurde 2020 ein erhöhter Anteil an Arbeitsunfällen mit einem ISS ≥16 festgestellt (2018: 10 %, 2019: 26,1 %, 2020: 44,4 %).
Diskussion
Eine Ausgangsbeschränkung führte zu keiner Reduktion verletzter- und intensivpflichtiger Patienten am untersuchten Zentrum. Auch unter einer landesweiten Ausgangsbeschränkung muss für dieses Patientenkollektiv eine ausreichende Menge an Intensiv- und OP-Kapazitäten vorgehalten werden. Die Bestätigung dieser Ergebnisse durch Auswertung nationaler Register steht noch aus.
Purpose
Evaluation of long-term outcome and toxicity of moderately hypofractionated radiotherapy using intensity-modulated radiotherapy (IMRT) with simultaneous integrated boost treatment planning and cone beam CT-based image guidance for localized prostate cancer.
Methods
Between 2005 and 2015, 346 consecutive patients with localized prostate cancer received primary radiotherapy using cone beam CT-based image-guided intensity-modulated radiotherapy (IG-IMRT) and volumetric modulated arc therapy (IG-VMAT) with a simultaneous integrated boost (SIB). Total doses of 73.9 Gy (n = 44) and 76.2 Gy (n = 302) to the high-dose PTV were delivered in 32 and 33 fractions, respectively. The low-dose PTV received a dose (D95) of 60.06 Gy in single doses of 1.82 Gy. The pelvic lymph nodes were treated in 91 high-risk patients to 45.5 Gy (D95).
Results
Median follow-up was 61.8 months. The 5‑year biochemical relapse-free survival (bRFS) was 85.4% for all patients and 93.3, 87.4, and 79.4% for low-, intermediate-, and high-risk disease, respectively. The 5‑year prostate cancer-specific survival (PSS) was 94.8% for all patients and 98.7, 98.9, 89.3% for low-, intermediate-, and high-risk disease, respectively. The 5‑year and 10-year overall survival rates were 83.8 and 66.3% and the 5‑year and 10-year freedom from distant metastasis rates were 92.2 and 88.0%, respectively. Cumulative 5‑year late GU toxicity and late GI toxicity grade ≥2 was observed in 26.3 and 12.1% of the patients, respectively. Cumulative 5‑year late grade 3 GU/GI toxicity occurred in 4.0/1.2%.
Conclusion
Moderately hypofractionated radiotherapy using SIB treatment planning and cone beam CT image guidance resulted in high biochemical control and survival with low rates of late toxicity.
Identification of new drug targets in adrenocortical carcinoma through targeted mRNA analysis

Adrenocortical carcinomas (ACC) are aggressive tumors associated with a heterogeneous but generally poor prognosis and limited treatment options for advanced stages. Despite promising molecular insights and improved understanding of ACC biology, efficient targeted therapies have not been identified yet. Thus, this study aims to identify potential new drug targets for a future personalized therapeutic approach.
RNA was isolated from 104 formalin-fixed paraffin-embedded tumor samples from ACC patients, 40 of those 104 cases proved to be suitable for further mRNA analyses according to the quality check of the extracted RNA. Gene expression of 84 known cancer drug targets was evaluated by quantitative real-time PCR using 5 normal adrenal glands as reference. Protein expression was investigated for selected candidate drug targets by immunohistochemistry in 104 ACC samples, 11 adenomas and 6 normal adrenal glands. Efficacy of an available inhibitor of the most promising candidate was tested by functional in vitro experiments in two ACC cell lines (NCI-H295R and MUC1) alone or in combination with other drugs.
Most frequently overexpressed genes were TOP2A, IGF2, CDK1, CDK4, PLK4 and PLK1. Nuclear immunostaining of CDK1, CDK4 and PLK1 significantly correlated with the respective mRNA expression. CDK4 was chosen as the most promising candidate for functional validation as it is actionable by FDA-approved CDK4/6 inhibitors. ACC samples with copy number gains at CDK4 locus presented significantly higher CDK4 expression levels. The CDK4/6 inhibitor palbociclib showed a concentration- and time- dependent reduction of cell viability in vitro, which was more pronounced in NCI-H295R than in MUC1 cells. This was in line with higher CDK4 expression at western blot analysis in NCI-H295R cells. Furthermore, palbociclib was applied in combination with dual IGFR/IR inhibitor linsitinib showing a synergistic effect on reducing cell viability. In conclusion, this proof-of-principle study confirmed RNA profiling to be useful to discover potential drug targets. Detected drug targets are suitable to be investigated by immunohistochemistry in the clinical setting. Moreover, CDK4/6 inhibitors are promising candidates for treatment of a subset of patients with tumors presenting CDK4 copy number gains and/or overexpression, while linsitinib might be an interesting combination partner in patients with both IGF2 and IGF1R overexpression.
These results are intended as a basis for a validation study in a prospective cohort, further evaluation in vivo in suitable mouse models or testing in patients with ACC in clinical trials are needed and might improve the future management of patients with ACC in terms of precision medicine.
In dieser Arbeit wird gezeigt, dass Fractalkin (CX3L1) keine Eisenregulation im Sinne des klassischen IRE/IRP-Systems aufweist. Zusätzlich wird die pathophysiologische Rolle der CX3CL1/CX3CR1-Achse in Megakaryozyten untersucht. Ferner wird die Eisenhomöostase während der megakaryopetischen Differenzierung erforscht.
