Alzheimer's disease (AD) is the most common form of dementia; however, mechanisms and biomarkers remain unclear. Here, we examined hippocampal CA4 and dentate gyrus subfields, which are less studied in the context of AD pathology, in post-mortem AD and control tissue to identify possible biomarkers. We performed mass spectrometry-based proteomic analysis combined with label-free quantification for identification of differentially expressed proteins. We identified 4,328 proteins, of which 113 showed more than 2-fold higher or lower expression in AD hippocampi than in control tissues. Five proteins were identified as putative AD biomarkers (MDH2, PCLO, TRRAP, YWHAZ, and MUC19 isoform 5) and were cross-validated by immunoblotting, selected reaction monitoring, and MALDI imaging. We also used a bioinformatics approach to examine upstream signalling interactions of the 113 regulated proteins. Five upstream signalling (IGF1, BDNF, ZAP70, MYC, and cyclosporin A) factors showed novel interactions in AD hippocampi. Taken together, these results demonstrate a novel platform that may provide new strategies for the early detection of AD and thus its diagnosis.
Most humans become infected with human cytomegalovirus (HCMV). Typically, the immune system controls the infection, but the virus persists and can reactivate in states of immunodeficiency. While substantial information is available on the contribution of CD8 T cells and antibodies to anti-HCMV immunity, studies of the T\(_{H}\)1, T\(_{H}\)2, and T\(_{H}\)17 subsets have been limited by the low frequency of HCMV-specific CD4 T cells in peripheral blood mononuclear cell (PBMC). Using the enzyme-linked Immunospot\(^{®}\) assay (ELISPOT) that excels in low frequency measurements, we have established these in a sizable cohort of healthy HCMV controllers. Cytokine recall responses were seen in all seropositive donors. Specifically, interferon (IFN)-\({\gamma}\) and/or interleukin (IL)-17 were seen in isolation or with IL-4 in all test subjects. IL-4 recall did not occur in isolation. While the ratios of T\(_{H}\)1, T\(_{H}\)2, and T\(_{H}\)17 cells exhibited substantial variations between different individuals these ratios and the frequencies were relatively stable when tested in samples drawn up to five years apart. IFN-\({\gamma}\) and IL-2 co-expressing polyfunctional cells were seen in most subjects. Around half of the HCMV-specific CD4 cells were in a reversible state of exhaustion. The data provided here established the T\(_{H}\)1, T\(_{H}\)2, and T\(_{H}\)17 characteristic of the CD4 cells that convey immune protection for successful immune surveillance against which reactivity can be compared when the immune surveillance of HCMV fails.
Biomedical research suffers from a dramatically poor translational success. For example, in ischemic stroke, a condition with a high medical need, over a thousand experimental drug targets were unsuccessful. Here, we adopt methods from clinical research for a late-stage pre-clinical meta-analysis (MA) and randomized confirmatory trial (pRCT) approach. A profound body of literature suggests NOX\(_{2}\) to be a major therapeutic target in stroke. Systematic review and MA of all available NOX\(_{2}\)\(^{-/y}\) studies revealed a positive publication bias and lack of statistical power to detect a relevant reduction in infarct size. A fully powered multi-center pRCT rejects NOX\(_{2}\) as a target to improve neurofunctional outcomes or achieve a translationally relevant infarct size reduction. Thus stringent statistical thresholds, reporting negative data and a MA-pRCT approach can ensure biomedical data validity and overcome risks of bias.
Zirkulierende Tumorzellen (CTCs) sind maligne Zellen, die in sehr geringer Anzahl im peripheren Blut von Tumorpatienten zu finden sind. Sie können entweder vom Primär-tumor oder von Metastasen in sekundären Organen stammen und sind in der Lage, sich nach dem Verlassen der Blutbahn in verschiedenen Geweben anzusiedeln und neue Me-tastasen auszubilden. Der Nachweis dieser CTCs im Blut ist mittlerweile zu einem viel-versprechenden und viel beforschtem Gebiet der Onkologie geworden. Das Vorhanden-sein von CTCs im Blut von Tumorpatienten kann ein früher Indikator für eine Metasta-sierung sein, als zuverlässiger Vorhersageparameter für die Prognose dienen und die Effektivität einer Therapie aufzeigen.
In der vorliegenden Arbeit wurde ein immunologisches Verfahren entwickelt, womit CTCs in venösem Vollblut schnell und einfach als EpCAM-und Zytokeratin-positive und CD45-negative Zellen nachgewiesen werden können. Dabei erfolgt nach einer Ly-se der Erythrozyten in der Probe eine Leukozytendepletion mittels MACS (magnetic-activated cell sorting) und die Identifikation der Tumorzellen mittels FACS-Analyse mit einer festgelegten Gatingstrategie. Mit diesem Verfahren wurden Blutproben von 42 Patienten mit metastasierten gastrointestinalen Tumoren und von 10 gesunden Normal-spendern auf die Anzahl von CTCs/3,75ml Blut untersucht. Dabei wiesen die Patienten annähernd signifikant (p=0,076) mehr CTCs auf als die Normalspender. In 43% der Fäl-le hatten die Patienten >2 CTCs/3,75ml Vollblut und galten damit als CTC-positiv. Pati-enten mit Kolorektalem Karzinom zeigten mit 71% den höchsten Anteil an CTC-Positivität. Bei den gesunden Normalspendern ließen sich in keinem Fall >2 CTCs nachweisen.
Nach einer Evaluation dieser Methode in einer weiteren Studie mit einem größeren Pati-enten- und Vergleichskollektiv könnte eine höhere Beweisebene erreicht werden, die den Einsatz in der klinischen Routine ermöglicht. Daneben sind eine weitere Optimie-rung des FACS-Verfahrens und eine Erweiterung des Antigenspektrums bei der CTC-Detektion zur Erhöhung der Sensitivität und Spezifität erforderlich. Patienten würden von diesem einfachen und wenig invasivem CTC-Detektionsverfahren vor allem im Hinblick auf eine Individualisierung der Therapie und auf das Vermeiden einer Überthe-rapie deutlich profitieren.
Polyneuropathien sind eine ätiologisch heterogene Erkrankung des peripheren Nervensystems. In bis zu 30% der Fälle ist eine Zuordnung zu einem bestimmten PNP Subtyp auch nach aufwändiger und zum Teil invasiver Diagnostik nicht möglich. Bislang fehlt ein diagnostischer Biomarker bei PNP, der z.B. bei der Unterscheidung zwischen einzelnen diagnostischen Subgruppen oder entzündlichen und nicht-entzündlichen Erkrankungsformen helfen könnte. In einer prospektiven Studie mit insgesamt 97 Patienten mit Neuropathien verschiedenster Ätiologie und 17 gesunden Kontrollpersonen erstellten wir Genexpressionsprofile von inflammatorischen Markern und Markern der Regeneration peripherer Nerven in Haut- und N. suralis-Biopsaten. Es wurden Inflammationsmarker (TAC1, CRMP2, AIF1, IL-6) und Marker, die in die Regeneration peripherer Nerven involviert sind (SCD, Netrin-1, DCC, UNC5H2, NEO1, Netrin-G1, Netrin-G2), mittels qRT-PCR untersucht. Alle Patienten erhielten eine N. suralis-Biopsie und/oder eine Hautbiopsie von Ober- beziehungsweise Unterschenkel. Weder in den Haut- noch in den N. suralis-Biopsaten konnten Unterschiede in der Genexpression dieser Marker zwischen einzelnen diagnostischen Subgruppen gefunden werden. Der Inflammationsmarker AIF1 war jedoch in Patienten-Hautproben sowohl proximal als auch distal höher exprimiert als bei gesunden Kontrollpersonen (p < 0,05 bzw. p < 0,01). Zudem fand sich in den Hautproben von PNP-Patienten eine deutlich reduzierte Genexpression von Regenerationsmarkern aus der Netrin-Familie verglichen mit den Hautproben gesunder Probanden (Netrin-1, DCC, UNC5H2, NEO1 sowie Netrin-G1 und G2; p < 0,05 bis p < 0,001). Ferner wies Netrin-1 in distalen Hautproben bei Patienten mit einer entzündlichen PNP eine niedrigere Genexpression auf, als bei Patienten mit einer nicht-entzündlichen Erkrankungsform (p < 0,05). Die Genexpression von NEO1 in distalen Hautproben war bei schmerzloser PNP und gesunden Kontrollpersonen höher als bei schmerzhafter PNP (p < 0,05). Sowohl eine Erhöhung bestimmter Inflammationsmarker als auch eine Verminderung von Regenerationsmarkern peripherer Nerven können bei der Pathophysiologie von Polyneuropathien involviert sein. Insbesondere Mitglieder der Netrin-Familie scheinen eine komplexe Rolle für das Axonwachstum, jedoch auch für entzündliche Prozesse zu spielen.
Background: The search for biomarkers in Parkinson's disease (PD) is crucial to identify the disease early and monitor the effectiveness of neuroprotective therapies. We aim to assess whether a gene signature could be detected in blood from early/mild PD patients that could support the diagnosis of early PD, focusing on genes found particularly altered in the substantia nigra of sporadic PD.
Results: The transcriptional expression of seven selected genes was examined in blood samples from 62 early stage PD patients and 64 healthy age-matched controls. Stepwise multivariate logistic regression analysis identified five genes as optimal predictors of PD: p19 S-phase kinase-associated protein 1A (odds ratio [OR] 0.73; 95% confidence interval [CI] 0.60-0.90), huntingtin interacting protein-2 (OR 1.32; CI 1.08-1.61), aldehyde dehydrogenase family 1 subfamily A1 (OR 0.86; 95% CI 0.75-0.99), 19 S proteasomal protein PSMC4 (OR 0.73; 95% CI 0.60-0.89) and heat shock 70-kDa protein 8 (OR 1.39; 95% CI 1.14-1.70). At a 0.5 cut-off the gene panel yielded a sensitivity and specificity in detecting PD of 90.3 and 89.1 respectively and the area under the receiving operating curve (ROC AUC) was 0.96. The performance of the five-gene classifier on the de novo PD individuals alone composing the early PD cohort (n = 38), resulted in a similar ROC with an AUC of 0.95, indicating the stability of the model and also, that patient medication had no significant effect on the predictive probability (PP) of the classifier for PD risk. The predictive ability of the model was validated in an independent cohort of 30 patients at advanced stage of PD, classifying correctly all cases as PD (100% sensitivity). Notably, the nominal average value of the PP for PD (0.95 (SD = 0.09)) in this cohort was higher than that of the early PD group (0.83 (SD = 0.22)), suggesting a potential for the model to assess disease severity. Lastly, the gene panel fully discriminated between PD and Alzheimer's disease (n = 29).
Conclusions: The findings provide evidence on the ability of a five-gene panel to diagnose early/mild PD, with a possible diagnostic value for detection of asymptomatic PD before overt expression of the disorder.
Das ON-Freezing ist ein seltenes, aber generell extrem schwer zu therapierendes Phänomen. Es betrifft Parkinson-Patienten mit und ohne THS.
Die derzeitige Literaturlage spiegelt wider, dass es unterschiedliche Strategien gibt, diesem Phänomen zu begegnen. Ein allgemeingültiges Therapiekonzept existiert dabei nicht. Für einige Patienten mit STN-THS konnte durch eine Reduktion der Stimulationsfrequenz eine Besserung der Gangstörung erzielt werden. Andere profitierten vom Einsatz sogenannter Interleaving-Protokolle mit gleichzeitiger Stimulation der Substantia nigra (Sn).
Im Vergleich zu anderen Arbeiten, die keine vorhersagbaren Parameter gefunden oder sich auf Symptome, Ausprägung der Subtypen und Erkrankungsdauer oder
den Zeitpunkt der Erkrankung konzentriert haben, verfolgten wir die Absicht, die Effekte der LF-Stim des STN auf Parkinson-Patienten mit Gangstörung und Freezing-Phänomen zu untersuchen und herauszufinden, ob man Gangparameter identifizieren kann, an Hand derer man das Ansprechen auf eine LF-Stim vorhersagen kann.
Unter der Einschränkung, dass die Zahl der Probanden unserer Studie sehr gering ist, haben wir herausgefunden, dass diejenigen Patienten besser auf eine LF-Stim ansprechen, die unter der Standard-HF-Stim eine signifikant höhere Ganggeschwindigkeit und eine größere Schrittlänge aufzeigen und nur ein intermittierendes Freezing haben.
Darüber hinaus zeigte sich ein besseres Ansprechen der LF-Stim bei Parkinson-Patienten mit akinetisch-rigidem Parkinson-Phänotyp.
Unsere Ergebnisse bestätigen die Annahme, dass sich L-Dopa additiv zur Stimulationstherapie bei manchen Parkinson-Patienten zusätzlich positiv auf die motorischen PD-Symptome auswirken kann. In Bezug auf die Verbesserung der Gangparameter zeigte sich in unseren Ergebnissen allerdings, dass L-Dopa eher eine untergeordnete Rolle spielt.
Aufgrund der niedrigen Anzahl von Respondern in unserer Studie lässt sich daher sicherlich noch keine allgemeingültige Regel ableiten. Es bedarf letztlich weiterer Studien mit größeren Untersuchungszahlen, um unsere Thesen zu stützen und abzusichern.
In jedem Fall wird aber das ON-Freezing auch weiterhin eine therapeutische Herausforderung bleiben.
Objective: To examine the effects of two different treatment approaches on the course of anorexia nervosa (AN) over time.
Methods: The subjects were 27 hospitalized AN patients (mean age: 14.91 years; mean BMI: 14.58; mean height: 163.56) . In our retrospective analysis we compared weight gain in two groups. While one group was treated with a standard oral refeeding protocol (historical control) through January 2013 (N=16), the second group (highly standardized refeeding protocol) received a high energy liquid nutrition and nutritional supplements including omega-3 fatty acids (N=11).
Results: On admission, the two groups were comparable in terms of height, weight, age and heart rate. At the end of our monitoring time frame of 25 days, weight gain was 121.4% higher in the highly standardized refeeding protocol group than in the historical control group (66.5 ±52.4 vs 147.3 ±55.7 grams/day; t-Test p=0.004; CI95%: 29.3-132.2). About 45% of our patients stated they were vegetarians at admission. However, we could not identify a vegetarian diet as a statistically significant negative prognostic factor for weight gain.
Discussion: The highly standardized refeeding protocol seems to be helpful in malnourished AN patients to improve weight gain without enhancing the risk of a refeeding syndrome. Because of an increasing energy turnover, caloric intake should be adjusted during refeeding.
Background
Salvage radiotherapy (SRT) is clinically established in prostate cancer (PC) patients with PSA persistence or biochemical relapse (BCR) after prior radical surgery. PET/CT imaging prior to SRT may be performed to localize disease recurrence. The recently introduced \(^{68}\)Ga-PSMA outperforms other PET tracers for detection of recurrence and is therefore expected also to impact radiation planning.
Forty-five patients with PSA persistence (16 pts) or BCR (29 pts) after prior prostatectomy, scheduled to undergo SRT of the prostate bed, underwent \(^{68}\)Ga-PSMA PET/CT. The median PSA level was 0.67 ng/ml. The impact of \(^{68}\)Ga-PSMA PET/CT on the treatment decision was assessed. Patients with oligometastatic (≤5 lesions) PC underwent radiotherapy (RT), with the extent of the RT area and dose escalation being based on PET positivity.
Results
Suspicious lesions were detected in 24/45 (53.3 %) patients. In 62.5 % of patients, lesions were only detected by 68Ga-PSMA PET. Treatment was changed in 19/45 (42.2 %) patients, e.g., extending SRT to metastases (9/19), administering dose escalation in patients with morphological local recurrence (6/19), or replacing SRT by systemic therapy (2/19). 38/45 (84.4 %) followed the treatment recommendation, with data on clinical follow-up being available in 21 patients treated with SRT. All but one showed biochemical response (mean PSA decline 78 ± 19 %) within a mean follow-up of 8.12 ± 5.23 months.
Conclusions
\(^{68}\)Ga-PSMA PET/CT impacts treatment planning in more than 40 % of patients scheduled to undergo SRT. Future prospective studies are needed to confirm this significant therapeutic impact on patients prior to SRT.
Background
The vascular type represents a very rare, yet the clinically most fatal entity of Ehlers-Danlos syndrome (EDS). Patients are often admitted due to arterial bleedings and the friable tissue and the altered coagulation contribute to the challenge in treatment strategies. Until now there is little information about clotting characteristics that might influence hemostasis decisively and eventually worsen emergency situations.
Results
22 vascular type EDS patients were studied for hemoglobin, platelet volume and count, Quick and activated partial thromboplastin time, fibrinogen, factor XIII, von Willebrand disease, vitamin D and platelet aggregation by modern standard laboratory methods. Results show a high prevalence of over 50 % for platelet aggregation disorders in vascular type EDS patients, especially for collagen and epinephrine induced tests, whereas the plasmatic cascade did not show any alterations. Additionally, more than half of the tested subjects showed low vitamin D serum levels, which might additionally affect vascular wall integrity.
Conclusion
The presented data underline the importance of detailed laboratory screening methods in vascular type EDS patients in order to allow for targeted application of platelet-interacting substances that might be of decisive benefit in the emergency setting.
Background
Multimodal treatment strategies – perioperative chemotherapy (CTx) and radical surgery – are currently accepted as treatment standard for locally advanced gastric cancer. However, the role of adjuvant postoperative CTx (postCTx) in addition to neoadjuvant preoperative CTx (preCTx) in this setting remains controversial.
Methods
Between 4/2006 and 12/2013, 116 patients with locally advanced gastric cancer were treated with preCTx. 72 patients (62 %), in whom complete tumor resection (R0, subtotal/total gastrectomy with D2-lymphadenectomy) was achieved, were divided into two groups, one of which receiving adjuvant therapy (n = 52) and one without (n = 20). These groups were analyzed with regard to survival and exclusion criteria for adjuvant therapy.
Results
Postoperative complications, as well as their severity grade, did not correlate with fewer postCTx cycles administered (p = n.s.). Long-term survival was shorter in patients receiving postCTx in comparison to patients without postCTx, but did not show statistical significance. In per protocol analysis by excluding two patients with perioperative death, a shorter 3-year survival rate was observed in patients receiving postCTx compared to patients without postCTx (3-year survival: 71.2 % postCTx group vs. 90.0 % non-postCTx group; p = 0.038).
Conclusion
These results appear contradicting to the anticipated outcome. While speculative, they question the value of post-CTx. Prospectively randomized studies are needed to elucidate the role of postCTx.
Tyrosine kinase inhibitors (TKI) have shown clinical effectiveness in iodine-refractory differentiated thyroid cancer (DTC). The corresponding role of serum thyroglobulin (Tg) in iodine-refractory DTC has not been investigated yet. 9 patients (3 female, 61 ± 8y) with progressive iodine-refractory DTC starting on lenvatinib were considered. Tumor restaging was performed every 2–3 months including contrast-enhanced computed tomography (CT, RECIST 1.1). Serum Tg was measured and compared to imaging findings. After treatment initiation, serum Tg levels dropped in all patients with a median reduction of 86.2%. During long-term follow-up (median, 25.2 months), fluctuations in Tg could be observed in 8/9 subjects. According to RECIST, 6/9 subjects achieved a partial response or stable disease with the remaining 3/9 experiencing progressive disease (2/3 with Tg levels rising above baseline). All of the patients with disease progression presented with a preceding continuous rise in serum Tg, whereas tumor marker oscillations in the subjects with controlled disease were only intermittent. Initiation of lenvatinib in iodine-refractory DTC patients is associated with a significant reduction in serum Tg levels as a marker of treatment response. In the course of treatment, transient Tg oscillations are a frequent phenomenon that may not necessarily reflect morphologic tumor progression.
Fabry disease is an X-linked lysosomal storage disorder due to impaired activity of alpha-galactosidase A with intracellular accumulation of globotriaosylceramide. Associated small fiber pathology leads to characteristic pain in Fabry disease. We systematically assessed sensory system, physical activity, metabolic parameters, and morphology of male and female mice with alpha-galactosidase A deficiency (Fabry ko) from 2 to 27 months of age and compared results with those of age- and gender-matched wild-type littermates of C57Bl/6J background. Results From the age of two months, male and female Fabry mice showed mechanical hypersensitivity (p < 0.001 each) compared to wild-type littermates. Young Fabry ko mice of both genders were hypersensitive to heat stimulation (p < 0.01) and developed heat hyposensitivity with aging (p < 0.05), while cold hyposensitivity was present constantly in young (p < 0.01) and old (p < 0.05) Fabry ko mice compared to wild-type littermates. Stride angle increased only in male Fabry ko mice with aging (p < 0.01) in comparison to wild-type littermates. Except for young female mice, male (p < 0.05) and female (p < 0.01) Fabry ko mice had a higher body weight than wild-type littermates. Old male Fabry ko mice were physically less active than their wild-type littermates (p < 0.05), had lower chow intake (p < 0.001), and lost more weight (p < 0.001) in a one-week treadmill experiment than wild-type littermates. Also, Fabry ko mice showed spontaneous pain protective behavior and developed orofacial dysmorphism resembling patients with Fabry disease.
Conclusions. Mice with alpha-galactosidase A deficiency show age-dependent and distinct deficits of the sensory system. alpha-galactosidase A-deficient mice seem to model human Fabry disease and may be helpful when studying the pathophysiology of Fabry-associated pain.
There is a largely divergent body of literature regarding the relationship between Epstein-Barr virus (EBV) infection and brain inflammation in multiple sclerosis (MS). Here, we tested MS patients during relapse (n = 11) and in remission (n = 19) in addition to n = 22 healthy controls to study the correlation between the EBV- and brain-specific B cell response in the blood by enzyme-linked immunospot (ELISPOT) and enzyme-linked immunosorbent assay (ELISA). Cytomegalovirus (CMV) was used as a control antigen tested in n = 16 MS patients during relapse and in n = 35 patients in remission. Over the course of the study, n = 16 patients were untreated, while n = 33 patients received immunomodulatory therapy. The data show that there was a moderate correlation between the frequencies of EBV- and brain-reactive B cells in MS patients in remission. In addition we could detect a correlation between the B cell response to EBV and disease activity. There was no evidence of an EBV reactivation. Interestingly, there was also a correlation between the frequencies of CMV- and brain-specific B cells in MS patients experiencing an acute relapse and an elevated B cell response to CMV was associated with higher disease activity. The trend remained when excluding seronegative subjects but was non-significant. These data underline that viral infections might impact the immunopathology of MS, but the exact link between the two entities remains subject of controversy.
Background
Fabry disease is characterized by a progressive deposition of sphingolipids in different organ systems, whereby cardiac involvement leads to death. We hypothesize that lysosomal storage of sphingolipids in the heart as occurring in Fabry disease does not reflect in higher cardiac lipid concentrations detectable by \(^1\)H magnetic resonance spectroscopy (MRS) at 3 Tesla.
Methods
Myocardial lipid content was quantified in vivo by \(^1\)H-MRS in 30 patients (12 male, 18 female; 18 patients treated with enzyme replacement therapy) with genetically proven Fabry disease and in 30 healthy controls. The study protocol combined \(^1\)H-MRS with cardiac cine imaging and LGE MRI in a single examination.
Results
Myocardial lipid content was not significantly elevated in Fabry disease (p = 0.225). Left ventricular (LV) mass was significantly higher in patients suffering from Fabry disease compared to controls (p = 0.019). Comparison of patients without signs of myocardial fibrosis in MRI (LGE negative; n = 12) to patients with signs of fibrosis (LGE positive; n = 18) revealed similar myocardial lipid content in both groups (p > 0.05), while the latter showed a trend towards elevated LV mass (p = 0.076).
Conclusions
This study demonstrates the potential of lipid metabolic investigation embedded in a comprehensive examination of cardiac morphology and function in Fabry disease. There was no evidence that lysosomal storage of sphingolipids influences cardiac lipid content as measured by \(^1\)H-MRS. Finally, the authors share the opinion that a comprehensive cardiac examination including three subsections (LGE; \(^1\)H-MRS; T\(_1\) mapping), could hold the highest potential for the final assessment of early and late myocardial changes in Fabry disease.
Zusammenfassung
Im Rahmen einer HIV-Infektion treten bei etwa der Hälfte aller Infizierten im Verlauf der Erkrankung neurokognitive Störungen auf. Zwar ist seit der Einführung der ART die Prävalenz der schweren HIV-Demenz zurückgegangen, aber vor allem mildere Formen sprechen nicht zufriedenstellend auf diese Therapie an und spielen auf Grund der insgesamt verbesserten Lebenserwartung von HIV-Patienten eine immer größere werdende Rolle. Um eine wirksame Therapie gegen HAND entwickeln zu können, sind detaillierte Kenntnisse über die Pathogenese und die Identifizierung von Risikofaktoren notwendig. In der vorliegenden Arbeit sollten daher drei unterschiedliche Faktoren auf ihren Zusammenhang mit HAND überprüft werden: der HIV-Protease Subtyp, die Höhe der Immunaktivierung und der DAT-Polymorphismus.
Hierfür standen Blut- und Plasmaproben von 112 HIV-Patienten und 30 Kontrollpersonen aus Tansania zur Verfügung, bei denen zuvor im Rahmen einer anderen medizinischen Doktorarbeit verschiedene neuropsychologische Tests durchgeführt worden waren. Zur Bestimmung des HIV-Protease Subtyps wurde die Virus-RNA aus dem Patienten-Plasma isoliert, in DNA umgeschrieben, gereinigt und anschließend sequenziert. Die Sequenz wurde mit Hilfe der Stanford University Database in Hinblick auf den HIV-Subtyp analysiert. Die Höhe der Immunaktivierung wurde durch Konzentrationsbestimmung der Immunaktivierungsmarker suPAR und human LBP mittels ELISA erfasst. Die Bestimmung des DAT-Polymorphismus erfolgte durch Isolation der Patienten-DNA aus den Blutproben, nachfolgender PCR und anschließender Agarosegelelektrophorese.
Die Untersuchung dieser drei Faktoren auf ihren Zusammenhang mit der neuropsychologischen Performance ergaben folgende Ergebnisse: 1. Der HIV-Protease Subtyp A und rekombinante Formen, die Subtyp A enthalten, gehen mit signifikant schlechteren Testergebnissen einher als die anderen HIV-Protease Subtypen. 2. Eine höhere Immunaktivierung korreliert ebenfalls mit schlechteren Testergebnissen. 3. Der DAT-Polymorphismus zeigt keine signifikanten Zusammenhänge mit HAND. Des Weiteren geht der HIV-Protease Subtyp C mit einer niedrigeren Immunaktivierung einher als die anderen Subtypen.
Diese Beobachtungen führen zu der Überlegung, dass der HIV-Protease Subtyp A sowie eine hohe periphere Immunaktivierung Risikofaktoren für die Entwicklung von HAND darstellen könnten und eventuell für die Pathogenese von Bedeutung sind. Der DAT-Polymorphismus scheint hierbei hingegen keine Rolle zu spielen. Bei dem Vorliegen des Subtyps A bzw. einer hohen Immunaktivierung sollte demnach über einen früheren Zeitpunkt für den Therapiebeginn mit ART nachgedacht werden, um die Entstehung von HAND zu verzögern oder sogar zu verhindern. Auf Grund der hohen Immunaktivierung als Risikofaktor für neuropsychologische Beeinträchtigungen sollte auch der Einsatz von immunmodulierenden Substanzen diskutiert werden. Die niedrigere Immunaktivierung, die bei einer Infektion mit dem HIV-Protease Subtyp C nachgewiesen werden konnte, könnte ein Hinweis auf eine niedrigere Pathogenität infolge einer Anpassung dieses Subtyps an den Menschen sein.
Da es sich bei all den durchgeführten Untersuchungen um Post-hoc-Analysen handelt, sind dringend weitere Studien zur Überprüfung der gefundenen Zusammenhänge notwendig.
Die von Proffit und Vig (1981) als Primary Failure of Eruption (PFE, Primäre Durchbruchstörung) klassifizierte Zahndurchbruchstörung resultiert klinisch häufig in schwergradigen Auswirkungen. Hierbei handelt es sich um Beeinträchtigungen des Wachstums des Alveolarfortsatzes, ebenso wie Dilazerationen, große vertikale Defekte und schwergradige lateral offene Bisse.
Die eindeutige Diagnostik und Abgrenzung der PFE von anderen Zahndurchbruchstörungen gestaltete sich bis zur Bestimmung der zugrunde liegenden Ursachen als sehr schwierig. Aufgrund von Fehldiagnosen kam es häufig zu Behandlungsmisserfolgen.
Um die PFE schneller und spezifischer diagnostizieren zu können, ist das Wissen über die zugrunde liegenden Mutationen des Parathormonrezeptor 1- Genes (PTHR1-Genes), welche bei PFE-Patienten isoliert wurden, von großer Bedeutung.
Im Zuge vorangegangener Studien wurden bereits einige Mutationen des PTHR1 als pathogen klassifiziert, hierzu zählt die PTHR1-Mutante W339*, eine Abbruchmutante, welche auf einem Basenaustausch beruht. Darüber hinaus liegen Daten zu potenziell pathogenen Genvariationen, wie die PTHR1-Mutante G452E, eine Aminosäureaustausch-Mutante, vor. Der Nachweis ihrer Pathogenität würde die Diagnosestellung sichern.
Um die Pathogenität der PTHR1-Variationen nachweisen zu können, wurde ihre RNA in X. laevis Oozyten injiziert. Der PTHR1 wurde zusammen mit mTRESK, einem Kaliumkanal, exprimiert und im Anschluss auf sein Verhalten bei Zugabe von 100 nM Parathormon (PTH) mit elektrophysiolgischen Messungen untersucht.
Im Rahmen der vorliegenden Arbeit konnte gezeigt werden, dass die bei nicht an PFE erkrankten Menschen vorkommende Variante des PTHR1 (PTHR1-WT) eine Aktivierung von 260,47% im Vergleich zu den Ausgangswerten unter einer physiologischen Lösung (ND96) zeigte. Im Gegensatz dazu konnte bei der bereits als pathogen klassifizierten PTHR1-Variation W339* kein signifikanter Anstieg der Aktivität unter PTH-Zugabe nachgewiesen werden. Für die potenziell pathogene PTHR1-Mutante G452E konnte ebenfalls keine signifikante Aktivitätssteigerung als Reaktion auf die Zugabe des Agonisten PTH nachzuweisen ermittelt werden.
Dies lässt die Schlussfolgerung zu, dass es sich bei der PTHR1-Mutante G452E ebenfalls um eine pathogene Variation des PTHR1-Genes handelt, genauso wie bei der als pathogen klassifizierten Variation W339* des PTHR1, da beide in den durchgeführten Messungen dasselbe Verhalten zeigen.
Die als Kontrollgruppe künstlich erzeugte Mutante G452A des PTHR1 zeigte hingegen eine signifikante Aktivierung von 91,02% im Vergleich zu den gemessenen Ausgangswerten unter physiolgischem ND96. Durch einen einfachen Aminosäureaustausch wurde die Basensequenz des Rezeptors so verändert, dass die Funktion trotz der Mutation wieder hergestellt werden konnte. Dies geschah durch den Einbau eines Alanins anstelle des natürlich vorkommenden Glycins. Im Gegensatz zu dem Einbau von Glutamat, bei der im Patientenkollektiv isolierten PTHR1-Mutante G452E, bei welcher die Funktionsfähigkeit nicht mehr vorliegt.
Die gemessene Aktivität ist zwar geringer als beim WT, legt aber nahe, dass es im Falle dieser künstlichen Mutation nicht zu einer Krankheitsausprägung kommt, da die Reaktion in ihrer Gesamtheit der des PTHR1-WT entspricht. Dies wird auch durch die signifikante Erhöhung des auswärts-gerichteten K+-Stromes deutlich, der sich analog zum gesunden PTHR1 verhält. . Es konnte somit die Funktionsfähigkeit der künstlichen PTHR1-Mutante G452A nachgewiesen werden. Die gesamten erzielten Ergebnisse waren durch die Abbildung von klinischen Befunden auf molekularer Ebene in Oozyten möglich. Durch die Kombination eines Kalium-Kanales mit dem krankheitsspezifischen Rezeptor konnte das Verhalten des Rezeptors anhand des mittels TEVC-Messungen ermittelten Verhaltens des Kalium-Kanales abgebildet werden.
Bei dem verwendeten Kalium-Kanal handelte es sich um mTRESK, welcher mit dem Parathormonrezeptor 1 zusammen exprimiert wurde. Durch die Zugabe des spezifischen Rezeptoragonisten PTH kam es bei den funktionsfähigen Variationen des Rezeptors zu einer Konformationsänderung des G-Proteins. Diese resultierte im weiteren Verlauf in einem Anstieg des intrazellulären Calcium-Spiegels und einer Aktivierung von Calcineurin. Die Dephosphorilierung des Kalium-Kanales mTRESK, welche zu einer Aktivitätssteigerung des Kanals führte, war die Folge.
Dies verdeutlicht, wie auch zukünftig durch die Kooexpression von krankheitsspezifischen Rezeptoren und elektrophysiologisch ableitbaren Strömen, die Bedeutungen und Auswirkungen von Mutationen auf molekularer Ebene funktionell nachgewiesen werden können.
Die vorliegende Arbeit erbringt somit unter Verwendung dieses Expressionssystems den Nachweis dafür, dass es sich bei der im Patientenkollektiv isolierten PTHR1-Mutante G452E um eine pathogene Variation des PTHR1-Genes handelt. Zudem konnten die vorangegangenen Ergebnisse, wonach es sich bei der ebenfalls im Patientenkollektiv isolierten PTHR1-Mutante W339* um eine pathogene Mutation handelt bestätigt werden.
Patienten mit diesen Genvariationen können somit eindeutig die Diagnose PFE erhalten und entsprechend zielführend therapiert werden.
High-resolution ultrasonography (HRUS) is an emerging new tool in the investigation of peripheral nerves. We set out to assess the utility of HRUS performed at lower extremity nerves in peripheral neuropathies. Nerves of 26 patients with polyneuropathies of different etiologies and 26 controls were investigated using HRUS. Patients underwent clinical, laboratory, electrophysiological assessment, and a diagnostic sural nerve biopsy as part of the routine work-up. HRUS was performed at the sural, tibial, and the common, superficial, and deep peroneal nerves. The superficial peroneal nerve longitudinal diameter (LD) distinguished best between the groups: patients with immune-mediated neuropathies (n = 13, including six with histology-proven vasculitic neuropathy) had larger LD compared to patients with non-immune-mediated neuropathies (p < 0.05) and to controls (p < 0.001). Among all subgroups, patients with vasculitic neuropathy showed the largest superficial peroneal nerve LD (p < 0.001) and had a larger sural nerve cross-sectional area when compared with disease controls (p < 0.001). Enlargement of the superficial peroneal and sural nerves as detected by HRUS may be a useful additional finding in the differential diagnosis of vasculitic and other immune-mediated neuropathies.
Background
Multiple sclerosis (MS) is an autoimmune disease of the central nervous system (CNS) characterized by inflammation, demyelination and axonal pathology. Myelin basic protein/proteolipid protein (MBP-PLP) fusion protein MP4 is capable of inducing chronic experimental autoimmune encephalomyelitis (EAE) in susceptible mouse strains mirroring diverse histopathological and immunological hallmarks of MS. Limited availability of human tissue underscores the importance of animal models to study the pathology of MS.
Methods
Twenty-two female C57BL/6 (B6) mice were immunized with MP4 and the clinical development of experimental autoimmune encephalomyelitis (EAE) was observed. Methylene blue-stained semi-thin and ultra-thin sections of the lumbar spinal cord were assessed at the peak of acute EAE, three months (chronic EAE) and six months after onset of EAE (long-term EAE). The extent of lesional area and inflammation were analyzed in semi-thin sections on a light microscopic level. The magnitude of demyelination and axonal damage were determined using electron microscopy. Emphasis was put on the ventrolateral tract (VLT) of the spinal cord.
Results
B6 mice demonstrated increasing demyelination and severe axonal pathology in the course of MP4-induced EAE. In addition, mitochondrial swelling and a decrease in the nearest neighbor neurofilament distance (NNND) as early signs of axonal damage were evident with the onset of EAE. In semi-thin sections we observed the maximum of lesional area in the chronic state of EAE while inflammation was found to a similar extent in acute and chronic EAE. In contrast to the well-established myelin oligodendrocyte glycoprotein (MOG) model, disease stages of MP4-induced EAE could not be distinguished by assessing the extent of parenchymal edema or the grade of inflammation.
Conclusions
Our results complement our previous ultrastructural studies of B6 EAE models and suggest that B6 mice immunized with different antigens constitute useful instruments to study the diverse histopathological aspects of MS.
In der vorliegenden Arbeit wurden vorsprachliche, kognitive und motorische Fähigkeiten von Säuglingen mit isolierter Sagittalnahtsynostose im Alter von 5 bis 7 Monaten mit einer gleichaltrigen Kontrollgruppe verglichen.
Die Sagittalnahtsynostose ist eine spezielle Form der Kraniosynostose, unter der man allgemein eine vorzeitige Verknöcherung von Schädelnähten versteht, die zur Entstehung pathologischer Kopfformen führt.
Die isolierte Sagittalnahtsynostose ist mit 190 Erkrankten pro eine Million Lebendgeburten die häufigste Variante der Kraniosynostose (Lajeunie et al., 1996). In verschiedenen Studien der letzten 15 Jahre wurden erhöhte Risiken für Beeinträchtigungen in den Bereichen Sprache, Kognition und Motorik bei Patienten mit isolierter Kraniosynostose beschrieben (Boltshauser et al., 2003; Chieffo et al., 2010; Da Costa et al., 2012; Kapp-Simon et al., 2007; Knight et al., 2014; Korpilahti et al., 2012; Magge et al., 2002; Mendonca et al., 2009; Shipster et al., 2003; Starr et al., 2007; Virtanen et al., 1999). Hier wurde untersucht, ob bereits präoperativ anhand vorsprachlicher Leistungen korrespondierende Auffälligkeiten bei Säuglingen mit Sagittalnahtsynostose nachweisbar sind.
Die Untersuchung der Probanden fand im Rahmen eines interdisziplinären Forschungsprojekts an der Universitätsklinik Würzburg unter dem Titel „Dreidimensionale stereophotogrammetrische Diagnostik des Schädels und Evaluierung der Therapie bei Kindern mit kraniofazialen Fehlbildungen unter Berücksichtigung der psychomotorischen Entwicklung“ statt. Im Rahmen dieser klinischen Pilotstudie werden die Kopfmaße der Säuglingsschädel mithilfe eines non-invasiven 3D-Scans erfasst und anschließend digital ausgewertet.
Für die Untersuchungen der vorsprachlichen Entwicklung wurden akustische Eigenschaften spontan geäußerter Komfortvokalisationen der Probanden analysiert. Dies geschah am Zentrum für vorsprachliche Entwicklung und Entwicklungsstörungen (ZVES) an der Poliklinik für Kieferorthopädie. Hierbei wurden unterschiedliche Grundfrequenzparameter sowie Laut- und Pausenlängen von Babbellauten untersucht.
Die Analyse der Säuglingsvokalisationen dient der Identifizierung potentieller neurophysiologischer Störungen. Soweit dem Verfasser der vorliegenden Dissertation bekannt, ist dies die erste Anwendung dieses Analyseverfahrens bei Säuglingen mit isolierter Sagittalnahtsynostose.
Insgesamt wurden ca. 2000 Vokalisationen von 14 Säuglingen mit isolierter Sagittalnahtsynostose sowie von 14 Kontrollkindern im Signalanalyselabor des ZVES mithilfe spezifischer Analyseprogramme ausgewertet. Dabei wurden ausschließlich Komfortvokalisationen ausgewählt; Weinen und vegetative Laute wurden ausgeschlossen.
Bei den Ergebnissen der Lautanalysen zeigten sich keine signifikanten Unterschiede zwischen den beiden Untersuchungsgruppen. Hinweise auf neurophysiologische Defizite der Säuglinge mit Sagittalnahtsynostose im Alter von 6 Monaten konnten aufgrund der Ergebnisse der Vorsprachlichen Diagnostik in der vorliegenden Untersuchung nicht gefunden werden. Es wurde lediglich eine größere Variabilität in der PG gefunden. Das könnte darauf hinweisen, dass einzelne Kinder ein Risiko für eine nachfolgende Entwicklungsverzögerung aufzeigen. Diese Annahme sollte durch Nachuntersuchung verifiziert bzw. widerlegt werden.
Neben der Sprachentwicklung wurden die Probanden auch bezüglich ihrer kognitiven und motorischen Leistungsfähigkeit getestet hier ausgewertet. Hierfür wurde die deutsche Fassung des Entwicklungstests Bayley Scales of Infant Development II verwendet.
Die Auswertung der kognitiven und motorischen Tests bestätigte die Ergebnisse der Lautanalysen. Es lagen keine signifikanten Unterschiede zwischen den beiden Probandengruppen vor. Die motorischen und kognitiven Fähigkeiten können im Mittel bei beiden Gruppen als gleichwertig angesehen werden. Es wurden zusätzlich Korrelationsanalysen durchgeführt, um einen möglichen Zusammenhang zwischen kognitiven, motorischen und zephalometrischen Parameter und den Lautparametern zu erkennen. Es lagen keine signifikanten Zusammenhänge zwischen den Parametern vor.
Die vorliegende Arbeit hat aufgrund der geringen Stichprobenzahl und angesichts des Pioniercharakters begrenzte Aussagekraft. Sie liefert aber eine geeignete Grundlage für weiterführende Studien an einem größeren Probandenkollektiv.
Background
X-chromosomal inheritance patterns and generally rare occurrence of Fabry disease (FD) account for mono-mutational hemizygous male and heterozygous female patients. Female mutation carriers are usually clinically much less severely affected, which has been explained by a suggested mosaicism in cell phenotype due to random allele shutdown. However, clinical evidence is scarce and potential additional effects in female gene carriers, which might account for specific clinical characteristics such as less severe chronic kidney disease, are yet unknown.
Case presentation
This article reports on a 45 year old female patient carrying the two alpha-galactosidase A gene mutations c.416A > G, p.N139S in exon 3 and c.708G > C, p.W236C in exon 5, but still showing only mild organ manifestations.
Conclusion
This current case highlights the importance of careful clinical characterization in patients with Fabry disease, who may show additional rare constellations and, therefore, are in need of personalized medicine. The impact of potential additional protective effects exceeding the presence of a non-pathogenic GLA allele in female gene carriers requires further investigation.
Patients with panic and post-traumatic stress disorders seem to show increased psychophysiological reactions to conditions of unpredictable (U) threat, which has been discussed as a neurobiological marker of elevated levels of sustained fear in these disorders. Interestingly, a recent study found that the right inferior frontal gyrus (rIFG) is correlated to the successful regulation of sustained fear during U threat. Therefore this study aimed to examine the potential use of non-invasive brain stimulation to foster the rIFG by means of anodal transcranial direct current stimulation (tDCS) in order to reduce psychophysiological reactions to U threat. Twenty six participants were randomly assigned into an anodal and sham stimulation group in a double-blinded manner. Anodal and cathodal electrodes (7 * 5 cm) were positioned right frontal to target the rIFG. Stimulation intensity was I = 2 mA applied for 20 min during a task including U threat conditions (NPU-task). The effects of the NPU paradigm were measured by assessing the emotional startle modulation and the skin conductance response (SCR) at the outset of the different conditions. We found a significant interaction effect of condition × tDCS for the SCR (F(2,48) = 6.3, p < 0.01) without main effects of condition and tDCS. Post hoc tests revealed that the increase in SCR from neutral (N) to U condition was significantly reduced in verum compared to the sham tDCS group (t(24) = 3.84, p < 0.001). Our results emphasize the causal role of rIFG for emotional regulation and the potential use of tDCS to reduce apprehension during U threat conditions and therefore as a treatment for anxiety disorders.
Background
Enhanced macromolecule biosynthesis is integral to growth and proliferation of cancer cells. Lipid biosynthesis has been predicted to be an essential process in cancer cells. However, it is unclear which enzymes within this pathway offer the best selectivity for cancer cells and could be suitable therapeutic targets.
Results
Using functional genomics, we identified stearoyl-CoA desaturase (SCD), an enzyme that controls synthesis of unsaturated fatty acids, as essential in breast and prostate cancer cells. SCD inhibition altered cellular lipid composition and impeded cell viability in the absence of exogenous lipids. SCD inhibition also altered cardiolipin composition, leading to the release of cytochrome C and induction of apoptosis. Furthermore, SCD was required for the generation of poly-unsaturated lipids in cancer cells grown in spheroid cultures, which resemble those found in tumour tissue. We also found that SCD mRNA and protein expression is elevated in human breast cancers and predicts poor survival in high-grade tumours. Finally, silencing of SCD in prostate orthografts efficiently blocked tumour growth and significantly increased animal survival.
Conclusions
Our data implicate lipid desaturation as an essential process for cancer cell survival and suggest that targeting SCD could efficiently limit tumour expansion, especially under the metabolically compromised conditions of the tumour microenvironment.
Background
Fabry-associated pain may be the first symptom of Fabry disease (FD) and presents with a unique phenotype including mostly acral burning triggerable pain attacks, evoked pain, pain crises, and permanent pain. We recently developed and validated the first Fabry Pain Questionnaire (FPQ) for adult patients. Here we report on the validation of the self-administered version of the FPQ that no longer requires a face-to-face interview but can be filled in by the patients themselves allowing more flexible data collection.
Methods
At our Würzburg Fabry Center for Interdisciplinary Treatment, Germany, we have developed the self-administered version of the FPQ by adapting the questionnaire to a self-report version. To do this, consecutive Fabry patients with current or past pain history (n = 56) were first interviewed face-to-face. Two weeks later patients’ self-reported questionnaire results were collected by mail (n = 55). We validated the self-administered version of the FPQ by assessing the inter-rater reliability agreement of scores obtained by supervised administration and self-administration of the FPQ.
Results
The FPQ contains 15 questions on the different pain phenotypes, on pain development during life with and without therapy, and on impairment due to pain. Statistical analysis showed that the majority of questions were answered in high agreement in both sessions with a mean AC1-statistic of 0.857 for 55 nominal-scaled items and a mean ICC of 0.587 for 9 scores.
Conclusions
This self-administered version of the first pain questionnaire for adult Fabry patients is a useful tool to assess Fabry-associated pain without a time-consuming face-to-face interview but via a self-reporting survey allowing more flexible usage.
Susac syndrome, a rare but probably underdiagnosed combination of encephalopathy, hearing loss, and visual deficits due to branch retinal artery occlusion of unknown aetiology has to be considered as differential diagnosis in various conditions. Particularly, differentiation from multiple sclerosis is often challenging since both clinical presentation and diagnostic findings may overlap. Optical coherence tomography is a powerful and easy to perform diagnostic tool to analyse the morphological integrity of retinal structures and is increasingly established to depict characteristic patterns of retinal pathology in multiple sclerosis. Against this background we hypothesised that differential patterns of retinal pathology facilitate a reliable differentiation between Susac syndrome and multiple sclerosis. In this multicenter cross-sectional observational study optical coherence tomography was performed in nine patients with a definite diagnosis of Susac syndrome. Data were compared with age-, sex-, and disease duration-matched relapsing remitting multiple sclerosis patients with and without a history of optic neuritis, and with healthy controls. Using generalised estimating equation models, Susac patients showed a significant reduction in either or both retinal nerve fibre layer thickness and total macular volume in comparison to both healthy controls and relapsing remitting multiple sclerosis patients. However, in contrast to the multiple sclerosis patients this reduction was not distributed over the entire scanning area but showed a distinct sectorial loss especially in the macular measurements. We therefore conclude that patients with Susac syndrome show distinct abnormalities in optical coherence tomography in comparison to multiple sclerosis patients. These findings recommend optical coherence tomography as a promising tool for differentiating Susac syndrome from MS.
To determine the role of striatal dopaminergic innervation on upper limb synergies during walking, we measured arm kinematics in 13 subjects with Parkinson disease. Patients were recruited according to several inclusion criteria to represent the best possible in vivo model of dopaminergic denervation. Of relevance, we included only subjects with normal spatio-temporal parameters of the stride and gait speed to avoid an impairment of upper limbs locomotor synergies as a consequence of gait impairment per se. Dopaminergic innervation of the striatum was measured by FP-CIT and SPECT. All patients showed a reduction of gait-associated arms movement. No linear correlation was found between arm ROM reduction and contralateral dopaminergic putaminal innervation loss. Still, a partition analysis revealed a 80% chance of reduced arm ROM when putaminal dopamine content loss was >47%. A significant correlation was described between the asymmetry indices of the swinging of the two arms and dopaminergic striatal innervation. When arm ROM was reduced, we found a positive correlation between upper-lower limb phase shift modulation ( at different gait velocities) and striatal dopaminergic innervation. These findings are preliminary evidence that dopaminergic striatal tone plays a modulatory role in upper-limb locomotor synergies and upper-lower limb coupling while walking at different velocities.
We analyzed rest tremor, one of the etiologically most elusive hallmarks of Parkinson disease(PD), in 12 consecutive PD patients during a specific task activating the locus coeruleus (LC) to investigate a putative role of noradrenaline (NA) in tremor generation and suppression. Clinical diagnosis was confirmed in all subjects by reduced dopamine reuptake transporter (DAT) binding values investigated by single photon computed tomography imaging (SPECT) with [\(^{123}\)I] N-\(\omega\)-fluoropropyl-2 \(\beta\)-carbomethoxy-3 \(\beta\)-(4-iodophenyl) tropane (FP-CIT). The intensity of tremor (i.e., the power of Electromyography [EMG] signals), but not its frequency, significantly increased during the task. In six subjects, tremor appeared selectively during the task. In a second part of the study, we retrospectively reviewed SPECT with FP-CIT data and confirmed the lack of correlation between dopaminergic loss and tremor by comparing DAT binding values of 82 PD subjects with bilateral tremor (n = 27), unilateral tremor (n = 22), and no tremor (n = 33). This study suggests a role of the LC in Parkinson tremor.
Background: Randomized controlled trials (RCT) on the treatment of severe space-occupying infarction of the middle cerebral artery (malignant MCA infarction) showed that early decompressive hemicraniectomy (DHC) is life saving and improves outcome without promoting most severe disablity in patients aged 18-60 years. It is, however, unknown whether the results obtained in the randomized trials are reproducible in a broader population in and apart from an academical setting and whether hemicraniectomy has been implemented in clinical practice as recommended by national and international guidelines. In addition, they were not powered to answer further relevant questions, e. g. concerning the selection of patients eligible for and the timing of hemicraniectomy. Other important issues such as the acceptance of disability following hemicraniectomy, the existence of specific prognostic factors, the value of conservative therapeutic measures, and the overall complication rate related to hemicraniectomy have not been sufficiently studied yet. Methods/Design: DESTINY-R is a prospective, multicenter, open, controlled registry including a 12 months follow-up. The only inclusion criteria is unilateral ischemic MCA stroke affecting more than 50% of the MCA-territory. The primary study hypothesis is to confirm the results of the RCT (76% mRS <= 4 after 12 months) in the subgroup of patients additionally fulfilling the inclusion cirteria of the RCT in daily routine. Assuming a calculated proportion of 0.76 for successes and a sample size of 300 for this subgroup, the width of the 95% CI, calculated using Wilson's method, will be 0.096 with the lower bound 0.709 and the upper bound 0.805. Discussion: The results of this study will provide information about the effectiveness of DHC in malignant MCA infarction in a broad population and a real-life situation in addition to and beyond RCT. Further prospectively obtained data will give crucial information on open questions and will be helpful in the plannig of upcomming treatment studies.
Die Interaktion mit Gehirnendothelzellen stellt ein zentraler Schritt in der Infektionspathogenese von Neisseria meningitidis dar. In dieser Promotionsarbeit konnte gezeigt werden, dass die Infektion von menschlichen Gehirnendothelzellen mit N. meningitidis zu einer transienten Aktivierung der sauren Sphingomyelinase (ASM) gefolgt von einer vermehrten Ceramidproduktion führt. Als Antwort auf die Infektion mit N. meningitidis kommt es zu einer vermehrten Präsentation der ASM und von Ceramiden an der äusseren Seite der Plasmamembran und zu einer Ausbildung von großen Ceramid-reichen Membran-Domänen, welche mit cortical plaque assoziierten Proteinen kolokalisieren. Bei dieser N. meningitids vermittelten Aktivierung der ASM spielt das bakterielle Aussenmembranprotein Opc sowie die Aktivierung der Phosphatidylcholin-spezifische Phospholipase C über die Interaktion von Opc mit Heparansulfat-Proteoglykane eine entscheidende Rolle. Die pharmakologische oder genetische Inhibition der ASM Funktion führt zu einer geringeren Invasivität der Meningokokken ohne dabei die Adhärenz zu beeinflussen. Im Einklang mit diesen Ergebnissen steht die Beobachtung, dass die geringere Invasivität von ausgewählten Isolaten des ST-11/ST-8 Komplex in menschlichen Gehirnendothelzellen direkt mit ihrer eingeschränkter Fähigkeit korreliert, die ASM zu aktivieren bzw. eine Ceramidproduktion zu induzieren. Schlussfolgernd ist die ASM Aktivierung und eine nachfolgende Ceramidproduktion essenziell für die Internalisierung von Opc-exprimierende Meningokokken in Gehirnendothelzellen und bietet einen Erklärungsansatz für die unterschiedliche Invasivität von verschiedenen N. meningitidis Stämmen.
Das Multiple Myelom (MM) ist eine durch monoklonale Vermehrung terminal differenzierter Antikörper-produzierender B-Lymphozyten (Plasmazellen) im Knochenmark charakterisierte maligne Krankheit, die sich v.a. in osteolytischen Knochendestruktionen, hämatopoetischer und Niereninsuffizienz äußert. Verbesserte Therapieansätze wie die Hochdosis-Chemotherapie mit Melphalan und anschließender autologer Stammzelltransplantation sowie die Einführung neuer pharmakologischer Substanzklassen (Proteasom-Inhibitoren, Cereblon-bindende Thalidomidderivate) führten zu einer Verlängerung der durchschnittlichen Überlebenszeit, für die meisten der Patienten ist die Erkrankung jedoch derzeit unheilbar. Die Erforschung neuer potenzieller therapeutischer Angriffspunkte auf Grund pathobiologischer Erkenntnisse bleibt daher unabdingbar. Ein Ansatz zur Verbesserung des Verständnisses der Pathogenese ist die funktionelle, molekulare und genetische Analyse des Signalnetzwerkes im MM. Im Zusammenhang mit diesem Konzept wurde entdeckt, dass wachstums-regulierende Signalwege in MM Zellen aktiviert oder dereguliert sind und zum Überleben und der Proliferation des Tumors beitragen. So konnte beispielsweise von unserer Arbeitsgruppe bereits gezeigt werden, dass onkogenes Ras essentiell zum Überleben der MM Zellen beiträgt. Da Ras derzeit mangels spezifischer Inhibitoren pharmakologisch nicht angreifbar ist, stellen weitere funktionelle Bestandteile des Signalweges eine potenzielle therapeutische Zielstruktur dar. Während die Blockade von MEK1/2 in MM Zellen keinen Einfluss auf das Überleben hatte, konnte durch die Blockade von Raf in ersten Tests unserer Arbeitsgruppe Apoptose hervorgerufen werden. Aus diesem Grund habe ich in der vorliegenden Arbeit zur Evaluation eines neuen Therapieansatzes die Rolle der Raf-abhängigen Signaltransduktion eingehend untersucht. Als Grundlage diente dabei die Hypothese, dass die Raf-Kinasen entscheidende Effektoren der durch onkogenes Ras vermittelten apoptotischen Effekte darstellen. In einem ersten Schritt konnte ich nachweisen, dass alle drei Raf-Isoformen (A-, B- und C-Raf) in humanen MM Zelllinien und in primären MM Zellen aktiviert sind. Mittels shRNA-vermittelter, Isoform-spezifischer Raf-Knockdown-Experimente konnte ich zeigen, dass nur ein simultaner Knockdown aller Isoformen, d.h. ein Pan-Raf-Knockdown, zu einer De-Phosphorylierung von MEK1/2 und ERK1/2 führte. Dieser Versuch ließ sich mittels pharmakologischer Raf-Inhibition, bei der ebenfalls nur eine Pan-Raf-Blockade zu einer Herunterregulation von MEK1/2 und ERK1/2 in MM Zellen führte, bestätigen. Das MEK/ERK-Modul stellte somit einen hervorragenden Surrogat- und Biomarker für die Pan-Raf-Aktivität dar. Im Gegensatz zur Blockade des MEK/ERK-Moduls führte eine Hemmung der Pan-Raf-Aktivität mittels shRNA oder pharmakologischer Inhibitoren in allen untersuchten Zelllinien und in der Mehrheit der primären MM Zellen zu einer starken Induktion von Apoptose. Da das Ansprechen auf eine Pan-Raf-Blockade nicht mit dem Ras-Mutationsstatus korrelierte, könnten die Raf-Kinasen eine von onkogenem Ras unabhängie Qualität als therapeutische Zielstruktur aufweisen. Zur Untersuchung möglicher MEK/ERK-unabhängiger Effektormechanismen der Pan-Raf-Inhibition habe ich die mRNA-basierten Genexpressionsprofile von INA-6 Zellen nach pharmakologischer Pan-Raf- oder MEK-Inhibition verglichen. Dabei führte die Pan-Raf-Inhibition zu einer Regulation von wesentlich mehr Genen, wobei sich auch die Art der regulierten Gene unterschied, darunter Gene mit tumorrelevanten Funktionen wie Regulation von Proliferation, Zellzyklus und Apoptose. Für eine dieser Gengruppen, die Gruppe der PI3K-abhängigen, mTOR-assoziierten Gene, konnte ich eine Regulation auch auf der Proteinebene nachweisen: die Phosphorylierungen von mTOR, p70S6K, Rb und AKT und die Expression von CyclinD1 und PDK1 waren nach Pan-Raf-Inhibition, nicht jedoch nach MEK-Blockade herunterreguliert. Dieses Ergebnis deutet auf eine Ko-Regulation der PI3K-abhängigen Signaltransduktion durch die Raf-kinasen hin. Mittels spezifischer PI3K-Inhibitoren ließ sich sowohl bei der Regulation der untersuchten Proteine als auch bei der Induktion von Apoptose eine deutliche Verstärkung der Pan-Raf-Inhibition in HMZL und in primären Zellen erzielen. Zusammengefasst zeigt diese Arbeit, dass die Pan-Raf-Blockade eine neue Therapiemöglichkeit darstellt, die durch Kombination mit einer PI3K/AKT-Inhibition noch verstärkt werden kann.
Background: The diagnostic and pathophysiological relevance of antibodies to aquaporin-4 (AQP4-Ab) in patients with neuromyelitis optica spectrum disorders (NMOSD) has been intensively studied. However, little is known so far about the clinical impact of AQP4-Ab seropositivity.
Objective: To analyse systematically the clinical and paraclinical features associated with NMO spectrum disorders in Caucasians in a stratified fashion according to the patients' AQP4-Ab serostatus.
Methods: Retrospective study of 175 Caucasian patients (AQP4-Ab positive in 78.3%).
Results: Seropositive patients were found to be predominantly female (p < 0.0003), to more often have signs of co-existing autoimmunity (p < 0.00001), and to experience more severe clinical attacks. A visual acuity of <= 0.1 during acute optic neuritis (ON) attacks was more frequent among seropositives (p < 0.002). Similarly, motor symptoms were more common in seropositive patients, the median Medical Research Council scale (MRC) grade worse, and MRC grades <= 2 more frequent, in particular if patients met the 2006 revised criteria (p < 0.005, p < 0.006 and p < 0.01, respectively), the total spinal cord lesion load was higher (p < 0.006), and lesions >= 6 vertebral segments as well as entire spinal cord involvement more frequent (p < 0.003 and p < 0.043). By contrast, bilateral ON at onset was more common in seronegatives (p < 0.007), as was simultaneous ON and myelitis (p < 0.001); accordingly, the time to diagnosis of NMO was shorter in the seronegative group (p < 0.029). The course of disease was more often monophasic in seronegatives (p < 0.008). Seropositives and seronegatives did not differ significantly with regard to age at onset, time to relapse, annualized relapse rates, outcome from relapse (complete, partial, no recovery), annualized EDSS increase, mortality rate, supratentorial brain lesions, brainstem lesions, history of carcinoma, frequency of preceding infections, oligoclonal bands, or CSF pleocytosis. Both the time to relapse and the time to diagnosis was longer if the disease started with ON (p < 0.002 and p < 0.013). Motor symptoms or tetraparesis at first myelitis and > 1 myelitis attacks in the first year were identified as possible predictors of a worse outcome.
HIV verursacht eine progressive Zerstörung des Immunsystems und führt zusätzlich durch Veränderungen im ZNS zu neurokognitiven Störungen (HIV-associated neurocognitive disorders, HAND). Die HIV-Infektion geht mit einer Dysfunktion von dopaminergen Signalwegen einher, die sich unter anderem in einer erhöhten Dopamin-Verfügbarkeit im Liquor von Therapie-naiven HIV-Patienten äußert. Der Grund für die Dysregulation der dopaminergen Signalwege in HIV-Patienten ist nicht geklärt. Aufgrund dessen war das Hauptziel dieser Arbeit die Identifizierung des pathogenetischen Mechanismus, der zu einer erhöhten Dopamin-Konzentration im Liquor von HIV-Patienten führt. Die primäre Hypothese war, dass die erhöhte Dopamin-Verfügbarkeit nicht durch das Virus selbst, sondern vielmehr durch die genetische Konstitution der HIV-Patienten hervorgerufen wird. Deshalb wurden Polymorphismen untersucht, die die dopaminerge Neurotransmission beeinflussen. Es wurde vermutet, dass a) verschiedene Genotypen dieser Polymorphismen in nicht-infizierten und HIV-infizierten Personen mit anderen Häufigkeiten auftreten, b) verschiedene Genotypen mit veränderten Dopamin-Verfügbarkeiten assoziiert sind, c) unterschiedliche Genotypen Auswirkungen auf Marker der Progression der HIV-Infektion haben und d) verschiedene Genotypen die Immunaktivierung beeinflussen. Dazu wurden in 190 HIV-infizierten und nicht-infizierten Teilnehmern unterschiedlicher Ethnien die Polymorphismen BDNF Val66Met, COMT Val108/158Met, DAT 3‘-UTR VNTR, DRD2 TaqIα, DRD3 Ser9Gly und DRD4 VNRT mit PCR, ggf. Restriktionsverdau und Agarose-Gelelektrophorese analysiert und die Expression des Dopamin-Transporters mit real time PCR bestimmt. Darüber hinaus wurden zur weiteren klinischen Charakterisierung die Immunmarker MCP-1, sCD14, suPAR und RANTES mit ELISA analysiert, da eine Erhöhung dieser Parameter mit einer beschleunigten HIV-Progression assoziiert ist. Die Bestimmung der T-Zell-Aktivierung (CD3/CD8/CD38/HLA-DR) wurde mit einer durchflusszytometrischen Analyse durchgeführt. In dieser Arbeit haben wir gezeigt, dass HIV-Patienten hochsignifikant häufiger homozygot für das 10-repeat Allel des Dopamin-Transporter-Polymorphismus sind als nicht-infizierte Personen (57,1 % bzw. 26,8 %, p = 0,001, OR = 3,93, 95 % CI 1,72 – 8,96, direkte logistische Regression). HIV-Patienten und nicht-infizierte Personen mit diesem Genotyp weisen eine signifikant höhere Dopamin-Verfügbarkeit im Liquor auf als Personen mit dem 9/10-Genotyp (p = 0,03) und eine signifikant geringere Expression des Dopamin-Transporters auf PBMCs (p = 0,05). Der DAT 10/10-Genotyp ist im Gegensatz zu anderen Genotypen in HIV-Patienten jedoch weder mit unterschiedlichen CD4+-Zellzahlen und Viruslasten noch mit einer veränderten Häufigkeit von HAND verbunden.
Zusätzlich weisen deutsche und südafrikanische nicht-infizierte und HIV-infizierte Personen mit dem DAT 10/10-Genotyp eine signifikant höhere MCP-1-Konzentration im Plasma auf als Personen mit anderen DAT-Genotypen (p = 0,0076). Keiner der Immunmarker ist mit der Dopamin-Verfügbarkeit assoziiert. Dennoch ist die Immunaktivierung in südafrikanischen HIV-Patienten im Vergleich zu nicht-infizierten Südafrikanern signifikant erhöht: HIV-Patienten zeigen im Vergleich zu nicht-infizierten Personen eine stärkere T-Zell-Aktivierung (p = 0,0001), eine erhöhte Plasma-Konzentration von MCP-1 (p = 0,0014), eine gesteigerte sCD14-Konzentration (p = 0,0004) und eine vermehrte suPAR-Konzentration im Plasma (p = 0,006). In der vorliegenden Arbeit konnte kein Nachweis erbracht werden, dass die erhöhte Immunaktivierung in den südafrikanischen HIV-Patienten durch die Koinfektion mit Echinoccocus oder durch genetische Polymorphismen bei Chemokinen hervorgerufen wird. Eine chronisch erhöhte Immunaktivierung stellt eine treibende Kraft für die Virusreplikation dar und kann letztendlich zu einer Erschöpfung des Immunsystems führen.
Der 10/10-Genotyp des DAT VNTR könnte einen Risiko-Faktor für die HIV-Infektion darstellen, da dieser eine erhöhte Dopamin-Verfügbarkeit nach sich zieht. Dopamin aktiviert HIV in chronisch infizierten T-Lymphoblasten und führt zudem zu einer erhöhten Expression und Sezernierung von TNF-α, das wiederum die Expression von HIV induziert. Diese Ergebnisse untermauern den Zusammenhang von Dopamin und HIV. Es ist jedoch nicht völlig geklärt, ob die erhöhte Dopamin-Konzentration ausschließlich durch den Genotyp hervorgerufen oder auch durch die HIV-Infektion begünstigt wird.
Zusammenfassend ist die Inzidenz von NNK bei Patienten mit einer chronischen
Nebennierenrindeninsuffizienz auch bei geschulten Patienten hoch. Das Auftreten der NNK
ist zudem mit einer erheblichen Morbidität und Mortalität verknüpft. In der heutigen Zeit
sterben weiterhin Patienten an den Folgen ihrer chronischen NNRI trotz einer adäquaten
Behandlung. Legt man die oben genannten Zahlen zugrunde, werden in den nächsten zehn
Jahren zwischen 5526 und 10647 Patienten an einer behandelbaren NNK versterben. Dies
macht die Notwendigkeit, die genauen Umstände und Ursachen einer NNK zu verstehen,
noch wichtiger. Die Analysen der Risikofaktoren einer NNK haben jedoch nur ein begrenztes
Potential, Patienten mit einem besonders hohen Risiko für das Auftreten von NKK zu
identifizieren. Patientenaufklärungen im Hinblick auf Dosisanpassungen der Glukokortikoide
in Stresssituationen werden die intravenösen Gaben von Glukokortikoiden nicht unnötig
machen, um eine drohende NNK zu verhindern. Anstrengungen um eine einheitliche
Patientenaufklärung mit dem Training der Selbstbehandlung mit parenteralem Hydrocortison
werden essentiell sein, um die NNK als Todesursache zu verhindern. Die aktuelle Studie
zeigt, dass die bisherigen Anstrengungen, eine einheitliche und breite Informationsgrundlage
zu vermitteln, nicht ausreichend sind. Denn auch bei medizinischem Fachpersonal besteht
anscheinend nach wie vor die Notwendigkeit einer intensiveren Schulung und Aufklärung. In
mehreren telefonischen Kontakten berichteten Patienten, dass sich ärztliche Kollegen nur
zurückhaltend auf eine ausreichend hohe intravenöse Cortisongabe einlassen konnten, in
einem Fall sogar, obwohl der betroffene Patient einen Notfallausweis bei sich trug. Auch in
der Laienpresse wurde von einem Fall berichtet, in dem laut Aussage der Patientin eine
Cortisonanpassung nicht ausreichend erfolgte.[40] Da die meisten tätigen Ärzte nur sehr
selten mit dem Krankheitsbild einer NNK konfrontiert sind, reagieren diese rezidivierend
nicht adäquat.[8] Auch zeigen sich die Symptome einer NNK häufig sehr unspezifisch,
weshalb es vielen Ärzten schwer fallen mag, diese zu erkennen. Da eine frühzeitige
Intervention für ein gutes Out Come jedoch unverzichtbar ist, sollten weitere Anstrengungen
gemacht werden, auch ärztliche Kollegen über diese Erkrankung und deren Behandlung
aufzuklären. Weiterhin zeigt sich auch die Schulung von Familie und Freunden als sinnvoll,
um das Aktionspotential im Falle einer akuten Verschlechterung der
Nebennierenrindenerkrankung zu verstärken.
423 Fragebögen waren für die Ersterhebung verfügbar, insgesamt schlossen 364 Patienten
(84%) das gesamte follow up über 2 Jahre ab. 767,5 Patientenjahre konnten über die Studie
erfasst werden. Innerhalb der Erhebungszeit wurde von 64 NNK berichtet. Dies entspricht
einer Häufigkeit von 8,3 Krisen/100 Patientenjahre. Als Hauptauslösefaktoren konnten
gastrointestinale Infektionen, Fieber und emotionaler Stress erkannt werden. Die Häufigkeit
entsprach 20%. Jedoch auch andere Stressoren konnten identifiziert werden. Hier zeigten sich
zum Beispiel Operationen, starke Schmerzen, Hitze, anstrengende körperliche Betätigung und
Schwangerschaft als ursächlich. In 7% der Fälle konnte bei einer plötzlichen
Verschlechterung des Zustandes keine auslösende Ursache gefunden werden. Es fand sich
jedoch, dass Patienten, die in ihrer Anamnese bereits eine oder mehrere NNK erlitten hatten,
ein höheres Risiko für eine erneute Entgleisung auswiesen. (Odds ratio 2,85, 95%
Konfidenzinterval 1.5–5.5, p 0,01) Andere Risikofaktoren konnten in der aktuellen Studie
nicht identifiziert werden. Während des Erhebungszeitraumes von zwei Jahren verstarben
insgesamt zehn Patienten. Vier dieser Todesfälle konnte einer NNK als Todesursache
zugeordnet werden. Dies entspricht einer Mortalitätshäufigkeit von 0,5/100 Patientenjahre.
Somit war in unserer Studie die NNK-assoziierte Mortalität 6% der NNK.
Unsere Studienteilnehmer wurden zu Beginn des Follow ups detailliert über die
Notwendigkeit von Dosisanpassungen und der Inanspruchnahme von professionellen Helfern
aufgeklärt. Dennoch zeigte sich im Vergleich zu anderen Studien mit 8,3 NNK/100
Patientenjahre keine reduzierte NNK-Häufigkeit. Jedoch konnte durchaus eine Reduktion der
NNK-Häufigkeit im Vergleich zu den Daten im Ersterhebungsbogen gezeigt werden. Neben
der Notwendigkeit, die Patientenaufklärung zu verbessern, zeigte die vorliegende Studie
jedoch auch, dass weitere Anstrengungen gemacht werden müssen, um das Vorgehen vor und
bei einer NNK weiterhin zu optimieren. So sind die genauen Umstände, die zu einer NNK
führen, bis heute noch nicht detailliert geklärt. Zwar konnten einige Risikofaktoren und
auslösende Situationen identifiziert werden, jedoch nicht die Frage, warum einige Patienten
eine NNK bekommen und Andere nicht. In diesem Zusammenhang besteht auch weiterhin die
Frage, warum einige Patienten gut auf eine Erhöhung der oralen Cortisondosis reagieren und
Andere trotz der Erhöhung in eine NNK kommen. Hieraus ergibt sich die Frage, ob es
Patienten gibt, die sensitiver auf das Cortison reagieren, und wenn dies der Fall ist, warum.
Hierfür könnte sprechen, dass es Patienten gibt, die rezidivierend in eine NNK kommen,
während Andere nie ein Krise erleiden. Auch wird es weiterhin Anstrengungen und neue
Strategien brauchen, eine schnellstmögliche Intervention im Falle einer Verschlechterung des
Allgemeinzustandes zu gewährleisten. Hier sind bessere, flächendeckende Aufklärungen für
medizinisches Fachpersonal und/oder die Verbesserung der mitgeführten Notfallkarten
notwendig. Eine Strategie, die Interventionszeit zu verkürzen, wäre der Ausbau der
Selbstinjektionen durch den betroffenen Patienten oder dessen Angehörige.
Microstructural Analysis of Peripheral Lung Tissue through CPMG Inter-Echo Time R2 Dispersion

Since changes in lung microstructure are important indicators for (early stage) lung pathology, there is a need for quantifiable information of diagnostically challenging cases in a clinical setting, e.g. to evaluate early emphysematous changes in peripheral lung tissue. Considering alveoli as spherical air-spaces surrounded by a thin film of lung tissue allows deriving an expression for Carr-Purcell-Meiboom-Gill transverse relaxation rates R-2 with a dependence on inter-echo time, local air-tissue volume fraction, diffusion coefficient and alveolar diameter, within a weak field approximation. The model relaxation rate exhibits the same hyperbolic tangent dependency as seen in the Luz-Meiboom model and limiting cases agree with Brooks et al. and Jensen et al. In addition, the model is tested against experimental data for passively deflated rat lungs: the resulting mean alveolar radius of RA = 31.46 \(\pm\) 13.15 \(\mu\)m is very close to the literature value (similar to 34 \(\mu\)m). Also, modeled radii obtained from relaxometer measurements of ageing hydrogel foam (that mimics peripheral lung tissue) are in good agreement with those obtained from mu CT images of the same foam (mean relative error: 0.06 \(\pm\) 0.01). The model's ability to determine the alveolar radius and/or air volume fraction will be useful in quantifying peripheral lung microstructure.
Owing to a high response rate, deep brain stimulation (DBS) of the ventral striatal area has been approved for treatment-refractory obsessive-compulsive disorder (tr-OCD). Many basic issues regarding DBS for tr-OCD are still not understood, in particular, the mechanisms of action and the origin of side effects. We measured prepulse inhibition (PPI) in treatment-refractory OCD patients undergoing DBS of the nucleus accumbens (NAcc) and matched controls. As PPI has been used in animal DBS studies, it is highly suitable for translational research. Eight patients receiving DBS, eight patients with pharmacological treatment and eight age-matched healthy controls participated in our study. PPI was measured twice in the DBS group: one session with the stimulator switched on and one session with the stimulator switched off. OCD patients in the pharmacologic group took part in a single session. Controls were tested twice, to ensure stability of data. Statistical analysis revealed significant differences between controls and (1) patients with pharmacological treatment and (2) OCD DBS patients when the stimulation was switched off. Switching the stimulator on led to an increase in PPI at a stimulus-onset asynchrony of 200 ms. There was no significant difference in PPI between OCD patients being stimulated and the control group. This study shows that NAcc-DBS leads to an increase in PPI in tr-OCD patients towards a level seen in healthy controls. Assuming that PPI impairments partially reflect the neurobiological substrates of OCD, our results show that DBS of the NAcc may improve sensorimotor gating via correction of dysfunctional neural substrates. Bearing in mind that PPI is based on a complex and multilayered network, our data confirm that DBS most likely takes effect via network modulation.
Viele Patienten, die an Schizophrenie erkrankt sind, zeigen dauerhafte Einschränkungen in sozial-kommunikativen und sozial-kognitiven Kompetenzen. Dies führt oft zu sozialem Rückzug, erschwert alltägliche zwischenmenschliche Interaktion und mindert die Lebensqualität der Patienten deutlich. Jene Einschränkungen sind bei Patienten mit Negativsymptomatik oder chronischen Zuständen besonders ausgeprägt und könnten einer Minderaktivierung im Spiegelneuronensystem unterliegen. Ziel dieser Studie war es, Korrelate von Defiziten in der sozialen Interaktion bei schizophrenen Patienten mit überwiegender Negativsymptomatik im Gegensatz zu gesunden Kontrollpersonen auf verschiedenen Ebenen darzustellen. Hierfür wurde die Fähigkeit zur sozialen Kognition anhand zweier verschiedener psychologischer Testverfahren erhoben und zudem die Gehirnaktivierung während alltagsähnlicher sozialer Interaktion mittels funktioneller Nahinfrarotspektroskopie gemessen.
Es konnte gezeigt werden, dass schizophrene Patienten mit vorherrschender Negativsymptomatik unter größeren Beeinträchtigungen zumindest in Teilaspekten von sozialer Kognition leiden als gesunde Kontrollpersonen. Hierbei steht Negativsymptomatik in Zusammenhang mit einer schlechteren Leistung im „Reading Mind in the Eyes Test“, was als „Undermentalizing“ angesehen werden kann. In Bezug auf die neurophysiologischen Messungen von Gehirnaktivität während alltagsähnlicher sozialer Interaktion konnte in der gesunden Kontrollgruppe eine fronto-temporo-parietale Aktivierung festgestellt werden. Hierbei steht insbesondere die Aktivität im Bereich des linken inferioren Parietallappens in Übereinstimmung mit den Ergebnissen zweier vorangegangener Studien (Egetemeir et al. 2011; Herrmann et al. 2015). In der Gruppe der schizophrenen Patienten dieser Studie jedoch zeigte sich keine während „Joint action“ spezifische Aktivität in temporo-parietalen Gehirnregionen. Ebenso war die Gehirnaktivität in den klassischen Spiegelneuronenarealen bei den Patienten im Vergleich zur Kontrollgruppe vermindert. Stattdessen kam es in der Patientengruppe zu einer erhöhten präfrontalen Gehirnaktivierung. Diese verschiedenartige Aktivierungsstrategie bei „Joint action“ kann als kompensatorische Gehirnaktivität interpretiert werden, die es den Patienten ermöglicht, soziale Interaktion erfolgreich zu bewältigen. Falls etwa die entscheidende Rolle während der Bewältigung der vorliegenden „Joint action“-Aufgabe in der Vermittlung visuell-räumlicher Aufmerksamkeitsprozesse durch den inferioren Parietallappen liegt (Herrmann et al. 2015), ist denkbar, dass diese Fähigkeit durch kompensatorische Vorgänge im präfrontalen Kortex übernommen werden kann. Da die Patienten dieser Studie zumeist seit längerer Zeit oder in chronisch residualem Zustand an Schizophrenie mit Negativsymptomatik litten, liegt es nahe, dass sich die kompensatorischen Strategien im Laufe der Zeit durch das alltägliche Leben ausreichend etablieren konnten. Die verminderte Aktivität in Spiegelneuronenarealen innerhalb der Patientengruppe untermauert das Konzept zur Krankheitsentstehung der Schizophrenie von Mehta und Kollegen, welches besagt, dass Gene und Umweltfaktoren ein möglicherweise angeboren defektes Spiegelneuronensystem beeinflussen, wobei erniedrigte Spiegelneuronenaktivität mit Defiziten in sozial kognitiven Einschränkungen und Negativsymptomatik einhergehe (Mehta et al. 2014a). Diese Zusammenhänge können jedoch im Rahmen dieser Studie lediglich vermutet und nicht objektiviert werden.
Durch die vorliegende Untersuchung konnte festgestellt werden, dass schizophrene Patienten mit Negativsymptomatik andere neuronale Strategien während alltagsähnlicher sozialer Interaktion nutzen als gesunde Personen, was einen weiteren Einblick in die neurobiologischen Grundlagen der Erkrankung erlaubt.
Background
Investigation of the expression of an intestinal stem cell marker in esophageal adenocarcinomas (EAC) with and without Barrett's Esophagus (BE), with respect to a cancer stem cell (CSC) hypothesis.
Materials and methods
Expression of a putative intestinal stem cell marker LgR5 was analyzed in esophageal cancer specimen (n = 70: 41 EAC with BE, 19 EAC without BE, and n = 10 esophageal squamous-cell carcinomas, ESCC) and in the adenocarcinoma cell line OE-33. Ki-67 and Cdx-2 were co-labelled with LgR5 in double staining experiments. Immunhistochemical expression results were confirmed by RT-PCR and correlated with tumor stage and five-year survival rates.
Results
LgR5was found expressed in 35 of 41 (85%) EAC with BE and in 16 of 19 (81%) EAC without BE. By contrast, LgR5 was not found to be expressed in ESCC. Quantification of immunolabeling showed 15% LgR5+ cells in EAC with BE, 32% LgR5+ cells in adjacent BE and 13% in EAC without BE. Immunofluorescence double staining experiments with LgR5 and Ki-67 revealed a subpopulation (~5%) of proliferating LgR+/Ki-67+ cells. On mRNA-level, expression of LgR5 was higher in BE in comparison to EAC (p = 0.0159). High levels of LgR5 expression in BE associated EAC were associated with poorer survival in univariate analysis.
Conclusion
The stem cell marker LgR5 is expressed in EAC, irrespective of association with BE, and appears to have negative impact on survival. The subset of proliferating LgR5+ cells (<5%) might resemble rapidly cycling CSCs, which needs to be substantiated in further investigations.
The proteasome of malaria parasites: A multi-stage drug target for chemotherapeutic intervention?

The ubiquitin/proteasome system serves as a regulated protein degradation pathway in eukaryotes, and is involved in many cellular processes featuring high protein turnover rates, such as cell cycle control, stress response and signal transduction. In malaria parasites, protein quality control is potentially important because of the high replication rate and the rapid transformations of the parasite during life cycle progression. The proteasome is the core of the degradation pathway, and is a major proteolytic complex responsible for the degradation and recycling of non-functional ubiquitinated proteins. Annotation of the genome for Plasmodium falciparum, the causative agent of malaria tropica, revealed proteins with similarity to human 26S proteasome subunits. In addition, a bacterial ClpQ/hslV threonine peptidase-like protein was identified. In recent years several independent studies indicated an essential function of the parasite proteasome for the liver, blood and transmission stages. In this review, we compile evidence for protein recycling in Plasmodium parasites and discuss the role of the 26S proteasome as a prospective multi-stage target for antimalarial drug discovery programs.
Background
Genome wide association studies reported two single nucleotide polymorphisms in ANK3 (rs9804190 and rs10994336) as independent genetic risk factors for bipolar disorder. Another SNP in ANK3 (rs10761482) was associated with schizophrenia in a large European sample. Within the debate on common susceptibility genes for schizophrenia and bipolar disorder, we tried to investigate common findings by analyzing association of ANK3 with schizophrenia, bipolar disorder and unipolar depression.
Methods
We genotyped three single nucleotide polymorphisms (SNPs) in ANK3 (rs9804190, rs10994336, and rs10761482) in a case-control sample of German descent including 920 patients with schizophrenia, 400 with bipolar affective disorder, 220 patients with unipolar depression according to ICD 10 and 480 healthy controls. Sample was further differentiated according to Leonhard's classification featuring disease entities with specific combination of bipolar and psychotic syndromes.
Results
We found no association of rs9804190 and rs10994336 with bipolar disorder, unipolar depression or schizophrenia. In contrast to previous findings rs10761482 was associated with bipolar disorder (p = 0.015) but not with schizophrenia or unipolar depression. We observed no association with disease entities according to Leonhard's classification.
Conclusion
Our results support a specific genetic contribution of ANK3 to bipolar disorder though we failed to replicate findings for schizophrenia. We cannot confirm ANK3 as a common risk factor for different diseases.
Background
"Negative affect" is one of the major migraine triggers. The aim of the study was to assess attentional biases for negative affective stimuli that might be related to migraine triggers in migraine patients with either few or frequent migraine and healthy controls.
Methods
Thirty-three subjects with frequent migraine (FM) or with less frequent episodic migraine, and 20 healthy controls conducted two emotional Stroop tasks in the interictal period. In task 1, general affective words and in task 2, pictures of affective faces (angry, neutral, happy) were used. For each task we calculated two emotional Stroop indices. Groups were compared using one-way ANOVAs.
Results
The expected attentional bias in migraine patients was not found. However, in task 2 the controls showed a significant attentional bias to negative faces, whereas the FM group showed indices near zero. Thus, the FM group responded faster to negative than to positive stimuli. The difference between the groups was statistically significant.
Conclusions
The findings in the FM group may reflect a learned avoidance mechanism away from affective migraine triggers.
Disorder-specific effects of polymorphisms at opposing ends of the Insulin Degrading Enzymegene

Background
Insulin-degrading enzyme (IDE) is the ubiquitously expressed enzyme responsible for insulin and amyloid beta (Aβ) degradation. IDE gene is located on chromosome region 10q23-q25 and exhibits a well-replicated peak of linkage with Type 2 diabetes mellitus (T2DM). Several genetic association studies examined IDE gene as a susceptibility gene for Alzheimer's disease (AD), however with controversial results.
Methods
We examined associations of three IDE polymorphisms (IDE2, rs4646953; IDE7, rs2251101 and IDE9, rs1887922) with AD, Aβ42 plasma level and T2DM risk in the longitudinal Vienna Transdanube Aging (VITA) study cohort.
Results
The upstream polymorphism IDE2 was found to influence AD risk and to trigger the Aβ42 plasma level, whereas the downstream polymorphism IDE7 modified the T2DM risk; no associations were found for the intronic variant IDE9.
Conclusions
Based on our SNP and haplotype results, we delineate the model that IDE promoter and 3' untranslated region/downstream variation may have different effects on IDE expression, presumably a relevant endophenotype with disorder-specific effects on AD and T2DM susceptibility.
Ziel des Projektes war es, die klinische Beeinträchtigung der chronisch mild und moderat hyponatriämen Patienten in einem longitudinalen Vergleich vor und nach Elektrolytkorrektur zu erfassen, um neue Therapieansätze in der Behandlung der als asymptomatisch eingestuften Erkrankung zu erlangen.
Die Studie unterteilte sich in einen diagnostischen Teil, in welchem Spontanurin und eine Blutprobe analysiert, sowie die klinische Anamnese erfasst wurden, und einen experimentellen Teil, in welchem eine kognitive und neuromuskuläre Testung erfolgte. Die kognitive Untersuchung erfasste die Reaktionsschnelligkeit und Konzentrationsfähigkeit der Patienten. Hierfür standen die validierte Testbatterie zur Aufmerksamkeitsprüfung (TAP 2.0 für Windows) und das Softwarepaket Wiener Testsystem 32.0 zur Verfügung. Die neuromuskuläre Testung erfolgte mit Hilfe der Messplattform Balance-X-Sensor. Das Vorgehen war standardisiert und reproduzierbar.
Zusätzlich wurde anhand eines Anamnesebogens die Ausprägung klinischer Zeichen einer symptomatischen Hyponatriämie erfragt (Kopfschmerzen, Konfusion, mentale Verlangsamung, Reizbarkeit, Streitsüchtigkeit, Desorientierung zu Zeit/Ort/Person, Müdigkeit, Leistungsfähigkeit, Schwindel, Übelkeit, Erbrechen, Gangunsicherheit, Stürze, Krämpfe) und abschließend Daten über die aktuelle Lebensqualität der Patienten mit Hilfe des SF-36 Fragebogens erhoben.
Im Anschluss an die Testungen wurden die Patienten dazu angewiesen, ihren Natriumhaushalt mithilfe einer individuellen Medikation, die unter Beachtung der aktuellen Behandlungsrichtlinien ausgewählt wurde, zu korrigieren. Eine Wiedervorstellung der Patienten erfolgte nach 14 Tagen um die Testungen im elektrolytkorrigierten Zustand zu wiederholen.
Ein longitudinaler Vergleich der Messergebnisse soll Aufschluss über das Ausmaß einer Veränderung bezüglich der Leistungsfähigkeit und dem Empfinden der Patienten geben. Um den Querschnittsvergleich mit einem gesunden Kontrollkollektiv zu ermöglichen, fanden die neuromuskulären und kognitiven Testungen ebenfalls mit normonatriämen, gesunden Probanden statt. Für den Querschnittsvergleich der Lebensqualitätsmessung diente die international anerkannte US-Kontrollpopulation aus dem Jahr 1998.
Die Studie wurde zur übersichtlicheren Darstellung in zwei separate Arbeiten aufgeteilt. Der vorliegende Teil befasst sich mit der neuromuskulären Gleichgewichtstestung, sowie mit der Evaluierung der subjektiv empfundenen Lebensqualität der Patienten vor und nach Elektrolytkorrektur.
Im Rahmen dieser Studie konnten Daten über das Gleichgewicht und die subjektive Lebensqualität von insgesamt 19 Patienten mit chronischer Hyponatriämie (121-133 mmol/l) erhoben werden.
Die Auswertung des Gleichgewichtstests mittels Balance-X-Sensor wies auf ein reduziertes Gleichgewichtsvermögen der Patienten nach Elektrolytkorrektur hin. Hierbei konnte in keinem der ermittelten Parameter (Kraftvektorfläche, Muskelleistungsfrequenz, Stehleistung) ein signifikanter Unterschied gezeigt werden. Vorangegangene Studien zum verbesserten Gleichgewichtsvermögen nach medikamentöser Einstellung der Serumnatriumkonzentration konnten somit nicht bestätigt werden. Einzelne Stehübungen wurden zudem von dem gesunden Kontrollkollektiv mit einem signifikant besseren Gleichgewicht, bei zugleich niedrigerer Standardabweichung, ausgeführt.
Die Erfassung der Lebensqualität erfolgte mit Hilfe des SF-36 Fragebogens. Die Patienten wiesen nach Elektrolytkorrektur insgesamt keine signifikant verbesserte Lebensqualität auf. Im Querschnittsvergleich mit einer gesunden Kontrollpopulation wies die Auswertung der Fragebögen auf eine insgesamt verminderte Lebensqualität, sowohl vor als auch nach Elektrolytkorrektur, hin.
Die vorliegende Studie zeigt eine körperliche Beeinträchtigung, sowie eine tendenziell verschlechterte subjektive Lebensqualität, chronisch mild hyponatriämer Patienten. Eine signifikante Verbesserung der Werte konnte nach Anheben der Serumnatriumkonzentration nicht festgestellt werden. Die gewonnenen Ergebnisse sollten in Folgestudien mit einem größeren Patienten- und Probandenkollektiv, einem krankheitsspezifischen Fragebogen, sowie einem doppelblinden Studienaufbau überprüft werden. Auch die Möglichkeit der Durchführung einer multizentrischen Studie sollte geprüft werden. Der SF-36 Fragebogen und der Balance-X-Sensor können hierbei als kostengünstige Messinstrumente den subjektiven und objektiven Erfolg der Therapiemaßnahme dokumentieren. Ein konsequentes Anheben der Serumnatriumkonzentration bis in den Normbereich liefert zudem eine bessere Vergleichbarkeit mit internationalen Studien.
Background: Use of FDG-PET/CT for staging and restaging of lymphoma patients is widely incorporated into current practice guidelines. Our aim was to prospectively evaluate the diagnostic performance of FDG-PET/MRI and WB-DW-MRI compared with FDG-FDG-PET/CT using a tri-modality PET/CT-MRI system.
Methods: From 04/12 to 01/14, a total of 82 FDG-PET/CT examinations including an additional scientific MRI on a tri-modality setup were performed in 61 patients. FDG-PET/CT, FDG-PET/MRI, and WB-DW-MRI were independently analyzed. A lesion with a mean ADC below a threshold of 1.2 x 10\(^{-3}\) mm\(^2\)/s was defined as positive for restricted diffusion. FDG-PET/CT and FDG-PET/MRI were evaluated for the detection of lesions corresponding to lymphoma manifestations according to the German Hodgkin Study Group. Imaging findings were validated by biopsy (n = 21), by follow-up imaging comprising CT, FDG-PET/CT, and/or FDG-PET/MRI (n = 32), or clinically (n = 25) (mean follow-up: 9.1 months).
Results: FDG-PET/MRI and FDG-PET/CT accurately detected 188 lesions in 27 patients. Another 54 examinations in 35 patients were negative. WB-DW-MRI detected 524 lesions, of which 125 (66.5 % of the aforementioned 188 lesions) were true positive. Among the 188 lesions positive for lymphoma, FDG-PET/MRI detected all 170 instances of nodal disease and also all 18 extranodal lymphoma manifestations; by comparison, WB-DW-MRI characterized 115 (67.6 %) and 10 (55.6 %) lesions as positive for nodal and extranodal disease, respectively. FDG-PET/MRI was superior to WB-DW-MRI in detecting lymphoma manifestations in patients included for staging (113 vs. 73), for restaging (75 vs. 52), for evaluation of high-(127 vs. 81) and low-grade lymphomas (61 vs. 46), and for definition of Ann Arbor stage (WB-DW-MRI resulted in upstaging in 60 cases, including 45 patients free of disease, and downstaging in 4).
Conclusion: Our results indicate that FDG-PET/CT and FDG-PET/MRI probably have a similar performance in the clinical work-up of lymphomas. The performance of WB-DW-MRI was generally inferior to that of both FDG-PET-based methods but the technique might be used in specific scenarios, e.g., in low-grade lymphomas and during surveillance.
One of the main shortcomings of interventional electrophysiology (EP) is its inability to generate sufficient soft tissue contrast for intra-procedural visualization of the myocardium and the surrounding tissue, using conventional imaging techniques. Interventional cardiovascular magnetic resonance imaging (MRI) aims at bringing about significant improvements to the complex and decisive EP interventions far beyond the capabilities of currently available supportive imaging techniques used to surmount the drawbacks of fluoroscopy, as MRI not only allows of precise three-dimensional exposure of the cardiovascular morphology, but also proves to be a promising technique exclusively suitable for direct visualization of arrhythmogenic substrate and therapeutic effects. Studying the function and malfunction of genes and proteins associated with inherited forms of peripheral neuropathies has provided multiple clues to our understanding of myelinated nerves in health and disease. Here, we have generated a mouse model for the peripheral neuropathy Charcot–Marie–Tooth disease type 4H by constitutively disrupting the mouse orthologue of the suspected culprit gene FGD4 that encodes the small RhoGTPase Cdc42-guanine nucleotide exchange factor Frabin. Lack of Frabin/Fgd4 causes dysmyelination in mice in early peripheral nerve development, followed by profound myelin abnormalities and demyelination at later stages. At the age of 60 weeks, this was accompanied by electrophysiological deficits. By crossing mice carrying alleles of Frabin/Fgd4 flanked by loxP sequences with animals expressing Cre recombinase in a cell type-specific manner, we show that Schwann cell-autonomous Frabin/Fgd4 function is essential for proper myelination without detectable primary contributions from neurons. Deletion of Frabin/Fgd4 in Schwann cells of fully myelinated nerve fibres revealed that this protein is not only required for correct nerve development but also for accurate myelin maintenance. Moreover, we established that correct activation of Cdc42 is dependent on Frabin/Fgd4 function in healthy peripheral nerves. Genetic disruption of Cdc42 in Schwann cells of adult myelinated nerves resulted in myelin alterations similar to those observed in Frabin/Fgd4-deficient mice, indicating that Cdc42 and the Frabin/Fgd4–Cdc42 axis are critical for myelin homeostasis. In line with known regulatory roles of Cdc42, we found that Frabin/Fgd4 regulates Schwann cell endocytosis, a process that is increasingly recognized as a relevant mechanism in peripheral nerve pathophysiology. Taken together, our results indicate that regulation of Cdc42 by Frabin/Fgd4 in Schwann cells is critical for the structure and function of the peripheral nervous system. In particular, this regulatory link is continuously required in adult fully myelinated nerve fibres. Thus, mechanisms regulated by Frabin/Fgd4–Cdc42 are promising targets that can help to identify additional regulators of myelin development and homeostasis, which may crucially contribute also to malfunctions in different types of peripheral neuropathies.
We describe monozygotic twins discordant for childhood leukemia and secondary thyroid carcinoma. We used bisulfite pyrosequencing to compare the constitutive promoter methylation of BRCA1 and several other tumor suppressor genes in primary fibroblasts. The affected twin displayed an increased BRCA1 methylation (12%), compared with her sister (3%). Subsequent bisulfite plasmid sequencing demonstrated that 13% (6 of 47) BRCA1 alleles were fully methylated in the affected twin, whereas her sister displayed only single CpG errors without functional implications. This between-twin methylation difference was also found in irradiated fibroblasts and untreated saliva cells. The BRCA1 epimutation may have originated by an early somatic event in the affected twin: approximately 25% of her body cells derived from different embryonic cell lineages carry one epigenetically inactivated BRCA1 allele. This epimutation was associated with reduced basal protein levels and a higher induction of BRCA1 after DNA damage. In addition, we performed a genome-wide microarray analysis of both sisters and found several copy number variations, i.e., heterozygous deletion and reduced expression of the RSPO3 gene in the affected twin. This monozygotic twin pair represents an impressive example of epigenetic somatic mosaicism, suggesting a role for constitutive epimutations, maybe along with de novo genetic alterations in recurrent tumor development.
Reliable biomarkers that can be used for early diagnosis and tracking disease progression are the cornerstone of the development of disease-modifying treatments for Parkinson’s disease (PD). The German Society of Experimental and Clinical Neurotherapeutics (GESENT) has convened a Working Group to review the current status of proposed biomarkers of neurodegeneration according to the following criteria and to develop a consensus statement on biomarker candidates for evaluation of disease-modifying therapeutics in PD. The criteria proposed are that the biomarker should be linked to fundamental features of PD neuropathology and mechanisms underlying neurodegeneration in PD, should be correlated to disease progression assessed by clinical rating scales, should monitor the actual disease status, should be pre-clinically validated, and confirmed by at least two independent studies conducted by qualified investigators with the results published in peer-reviewed journals. To date, available data have not yet revealed one reliable biomarker to detect early neurodegeneration in PD and to detect and monitor effects of drug candidates on the disease process, but some promising biomarker candidates, such as antibodies against neuromelanin, pathological forms of α-synuclein, DJ-1, and patterns of gene expression, metabolomic and protein profiling exist. Almost all of the biomarker candidates were not investigated in relation to effects of treatment, validated in experimental models of PD and confirmed in independent studies.
In previous studies of a genetic isolate, we identified significant linkage of attention deficit hyperactivity disorder (ADHD) to 4q, 5q, 8q, 11q and 17p. The existence of unique large size families linked to multiple regions, and the fact that these families came from an isolated population, we hypothesized that two-locus interaction contributions to ADHD were plausible. Several analytical models converged to show significant interaction between 4q and 11q (P<1 × 10−8) and 11q and 17p (P<1 × 10−6). As we have identified that common variants of the LPHN3 gene were responsible for the 4q linkage signal, we focused on 4q–11q interaction to determine that single-nucleotide polymorphisms (SNPs) harbored in the LPHN3 gene interact with SNPs spanning the 11q region that contains DRD2 and NCAM1 genes, to double the risk of developing ADHD. This interaction not only explains genetic effects much better than taking each of these loci effects by separated but also differences in brain metabolism as depicted by proton magnetic resonance spectroscopy data and pharmacogenetic response to stimulant medication. These findings not only add information about how high order genetic interactions might be implicated in conferring susceptibility to develop ADHD but also show that future studies of the effects of genetic interactions on ADHD clinical information will help to shape predictive models of individual outcome.
The objective of this study was to use data from a noninterventional study to evaluate the effectiveness of adalimumab in rheumatoid arthritis (RA) patients during routine clinical practice and to explore the potential impact of patient and disease characteristics in response to adalimumab therapy. A total of 2,625 RA patients with specified data at baseline (prior to initiating adalimumab treatment) and 12 months entered this study between April 2003 and March 2009. We evaluated response to adalimumab therapy and conducted stepwise regression and subgroup analyses of factors influencing therapeutic response. During the 1-year adalimumab treatment period, disease activity decreased from a baseline mean disease activity score-28 joints (DAS28) of 5.9–3.9, while functional capacity improved from 59.0 to 68.4 Funktionsfragebogen Hannover (FFbH) percentage points. In multivariate regression models, high baseline DAS28 was the strongest positive predictor for decrease in disease activity, and high baseline functional capacity was associated with reduced gains in functional capacity. Male gender was a positive predictor of therapeutic response for both disease activity and functional capacity, while older age and multiple previous biologics were associated with a reduced therapeutic response. Subset analyses provided further support for the impact of baseline DAS28, FFbH, and prior biologic therapy on therapeutic response during treatment. We conclude that treatment with adalimumab leads to decreased disease activity and improved function during routine clinical practice. Patients with high disease activity and low functional capacity are particularly benefitted by adalimumab therapy.
Cellular and cytokine-dependent immunosuppressive mechanisms of grm1-transgenic murine melanoma

Grm1-transgenic mice spontaneously develop cutaneous melanoma. This model allowed us to scrutinize the generic immune responses over the course of melanoma development. To this end, lymphocytes obtained from spleens, unrelated lymph nodes and tumor-draining lymph nodes of mice with no evidence of disease, and low or high tumor burden were analyzed ex vivo and in vitro. Thereby, we could demonstrate an increase in the number of activated CD4\(^+\) and CD8+ lymphocytes in the respective organs with increasing tumor burden. However, mainly CD4\(^+\) T cells, which could constitute both T helper as well as immunosuppressive regulatory T cells, but not CD8\(^+\) T cells, expressed activation markers upon in vitro stimulation when obtained from tumor-bearing mice. Interestingly, these cells from tumor-burdened animals were also functionally hampered in their proliferative response even when subjected to strong in vitro stimulation. Further analyses revealed that the increased frequency of regulatory T cells in tumor-bearing mice is an early event present in all lymphoid organs. Additionally, expression of the immunosuppressive cytokines TGF-β1 and IL-10 became more evident with increased tumor burden. Notably, TGF-β1 is strongly expressed in both the tumor and the tumor-draining lymph node, whereas IL-10 expression is more pronounced in the lymph node, suggesting a more complex regulation of IL-10. Thus, similar to the situation in melanoma patients, both cytokines as well as cellular immune escape mechanisms seem to contribute to the observed immunosuppressed state of tumor-bearing grm1-transgenic mice, suggesting that this model is suitable for preclinical testing of immunomodulatory therapeutics.
