Fear and anxiety disorders – interaction of AVP and OXT brain systems with the serotonergic system

Anxiety disorders pose a great burden onto society and economy and can have devastating consequences for affected individuals. Treatment options are still limited to psychopharmacotherapy originally developed for the treatment of depression and behavioral therapy. A combination of genetic traits together with aversive events is most likely the cause of these diseases. Gene x environment studies are trying to find a link between genetic traits and specific negative circumstances. In a first study, we focused on social anxiety disorder (SAD), which is the second most-common anxiety disorder after specific phobias. We used a social fear conditioning (SFC) paradigm, which is able to mimic the disease in a mouse model. We wanted to investigate protein levels, as well as mRNA expression of immediate early genes (IEGs), to determine brain areas affected by the paradigm. We also included genes of the vasopressin (AVP)-, oxytocin (OXT)-, neuropeptide Y (NPY)-, and the serotonin system, to investigate the effects of SFC on neurotransmitter gene expression levels in brain regions related to social as well as fear-related behavior. AVP and OXT regulate a lot of different social and anxiety-related behaviors, both positive and negative. Finding a link between different neurotransmitter systems in the development of anxiety disorders could help to identify potential targets for new treatment approaches, which are desperately needed, because the rate of patients not responding to available treatment is very high.
We were able to show altered gene expression of the IEGs cFos and Fosl2, as well as a change in number and density of cFOS-positive cells in the dorsal hippocampus, indicating an influence of SFC on neuronal activity. Our results reveal a possible involvement of anterior dentate gyrus (DG), as well as cornu ammonis area 1 (CA1) and CA3 in the dorsal hippocampus during the expression of social fear. Contrary to our hypothesis, we were not able to see changes in neuronal activity through expression changes of IEGs in the amygdala. Significant higher IEG immunoreactivity and gene expression in the dorsal hippocampus of animals without fear conditioning (SFC-), compared to animals with fear conditioning (SFC+), indicate an involvement of different hippocampal regions in two possible scenarios. Either as elevated gene expression in SFC- animals compared to SFC+ animals or as reduction in SFC+ animals compared to SFC- animals. However, this question cannot be answered without an additional control of basal IEG-activity without social interaction. The NPY system in general and the neuropeptide y receptor type 2 in particular seem to be involved in regulating the response to social fear, mostly through the septum region. In addition to that, a possible role for the induction of social fear response could be identified in the serotonergic system and especially the serotonin receptor 2a of the PVN.
In a second study we focused on changes in the serotonergic system. A polymorphism in the human serotonin transporter (5-HTT) gene is associated with higher risks for the development of anxiety disorders. This makes the 5-HTT a widely used target to study possible causes and the development of anxiety disorders. In mice, a genetically induced knockout of the 5-Htt gene is associated with increased anxiety-like behavior. High amounts of stress during pregnancy, also known as prenatal stress, significantly increase the risk to develop psychiatric disorders for the unborn child. We utilized a prenatal stress paradigm in mice heterozygous for the 5-Htt gene. Some of the animals which had been subjected to prenatal stress showed noticeably “unsocial” interaction behavior towards conspecifics. Again, we were searching for links between the serotonergic system and AVP- and OXT systems. Through quantitative gene expression analysis, we were able to show that both AVP and OXT neuromodulator systems are affected through prenatal stress in female mice, but not in male mice. The 5-Htt genotype seems to be only slightly influential to AVP, OXT or any other neurotransmitter system investigated. Gene expression of AVP and OXT brain systems is highly influenced through the estrous cycle stages of female mice. Additionally, we analyzed the AVP and OXT neuropeptide levels of mice with different 5-Htt genotypes and in both sexes, in order to see whether the production of AVP and OXT is influenced by 5-Htt genotype. On neuropeptide level, we were able to identify a sex difference for vasopressin-immunoreactive (ir) cells in the PVN, with male mice harboring significantly more positive cells than female mice.
Diese Studie evaluiert die Mundgesundheit von Hypophosphatasie (HPP)-Patienten und beinhaltet die Untersuchungen folgender Kenngrößen: erster Milchzahnverlust, Anzahl fehlender Zähne, DMF/T-Index (Kariös/Fehlend/Gefüllt), Blutung auf Sondierung (BOP), Sondierungstiefen (PPD), klinisches Attachmentniveau (CAL), Zahnlockerungen und kombinierter CDC/AAP Parodontal-Index. Die Daten wurden zwischen HHP-Patienten mit einer und zwei Mutationen auf dem für die alkalische Phosphatase kodierenden Genabschnitt (ALPL-Gen), sowie mit den Ergebnissen der DMS V verglichen, welche die Mundgesundheit der deutschen Allgemeinbevölkerung repräsentiert. 80 Patienten wurden untersucht; 64 von ihnen waren weiblich, 18 männlich. Das Mittlere Alter lag bei 46,4 Jahren. 55 Studienteilnehmer wiesen eine Mutation und 18 zwei Mutationen des ALPL-Gens auf (n=7 keine Testung). Patienten, die von zwei Mutationen des für die alkalische Phosphatase kodierenden Genabschnitts betroffen waren, zeigten ein höheres Risiko für Parodontitis und Zahnverlust als HPP-Patienten mit nur einer Mutation, sowie im Vergleich mit der deutschen Allgemeinbevölkerung. Allerdings stiegen auch bei Patienten mit nur einer Mutation mit zunehmendem Lebensalter die Ausprägungen parodontaler Erkrankungen, sowie die Zahnverlustrate im Vergleich zur Durchschnittsbevölkerung in Deutschland.
The StrongPaed study in the paediatric ward of a referral hospital in Mwanza in the lake region of Tanzania showed the prevalence of S. stercoralis, G. lamblia, E. histolytica and E. dispar as well as of other intestinal parasites with various diagnostic methods.
The prevalence of S. stercoralis was 2-10 % depending on the diagnostic methods used. There were no symptomatic infections but only carriage of the nematode. The positive results differed greatly depending on the performed diagnostic methods. None of the diagnostics showed satisfying results, neither in sensitivity and specificity nor in feasibility for this population in an endemic region in sub-Saharan Africa. PCR and microscopy were limited by the low amount of examined stool samples and by the resulting lack of sensitivity. Stool cultures were limited by time-consuming procedures and mainly by the problem of differentiation from hookworm and the resulting lack of specificity. ELISA was limited by the need of blood samples and also by poor specificity in the ELISA used.
The prevalence of G. lamblia was high, but mostly only carriage and not symptomatic infections was seen. No E. histolytica was detected, but 8.5 % samples were positive for E. dispar. Among the performed diagnostics, the rapid test showed sufficient results. It showed better sensitivity than microscopy and is cheaper and more feasible than PCR. Differentiation between E. histolytica and E. dispar was only possible with qPCR performed in Germany.
More children were positive for intestinal parasites from rural than from urban areas. The profession of the parents working as farmers was a risk factor for intestinal parasitic infections. Hygienic living conditions such as access to tap water and flush toilets at home were preventive for intestinal parasitic infections in children.
Schimmelpilze können in Abhängigkeit des Immunstatus und der Vorerkrankungen betroffener Patienten unterschiedliche Krankheitsbilder wie Hypersensitivitäts-erkrankungen oder lebensbedrohliche invasive Infektionen hervorrufen. Da die Diagnosestellung dieser Erkrankungen mitunter komplex und insensitiv ist, sollten im Rahmen dieser Arbeit unterschiedliche Ansätze neuer diagnostischer Assays untersucht werden.
In den letzten Jahren wurden Assays entwickelt, die auf Basis durchflusszytometrisch quantifizierter Pilz-spezifischer T-Zellen aus peripherem Blut einen supportiven Biomarker zur Diagnostik invasiver Mykosen liefern könnten. Da die hierfür isolierten T-Zellen anfällig gegenüber präanalytischer Lagerzeiten und immunsuppressiver Medikation sind, wurden hier Protokolloptimierungen vorgenommen, um anhand eines Vollblut-basierten Assays mit zusätzlicher CD49d-Kostimulation diesen Limitationen entgegen zu wirken. In einer Studie an gesunden Probanden konnte dabei gezeigt werden, dass die Kombination der Durchflusszytometrie mit ausgewählten Zytokin-Messungen (IL-5, IL-10 und IL-17) zu einer verbesserten Erkennung vermehrt Schimmelpilz-exponierter Personen beitragen könnte. Neben Infektionen könnten dabei im umwelt- und arbeitsmedizinischen Kontext Polarisationen der T-Zell-Populationen detektiert werden, welche mit Sensibilisierungen und Hypersensitivität assoziiert werden.
Zusätzlich wurde ein in vitro Transwell® Alveolarmodell zur Simulation pulmonaler Pilzinfektionen für Erreger der Ordnung Mucorales adaptiert, durch Reproduktion wichtiger Merkmale der Pathogenese von Mucormykosen validiert, und für Untersuchungen der Immunpathologie und Erreger-Invasion verwendet. Das Modell wurde anschließend zur in vitro Evaluation von radioaktiv markiertem Amphotericin B mit 99mTc oder 68Ga als nuklearmedizinischen Tracer verwendet. Die untersuchten Schimmelpilze zeigten dabei eine zeit- und dosis-abhängige Aufnahme der Tracer, während bakteriell infizierte Proben nicht detektiert wurden. Die erhobenen Daten dokumentieren ein vielversprechendes Potenzial von Amphotericin B-basierten Tracer, das in zukünftigen in vivo Studien weiter evaluiert werden sollte.
Der Zusammenhang von neuropathischem Schmerz mit einer gestörten Blut-Nerven-
Schranke (BNS) ist bekannt. Die BNS wird durch Tight Junction Proteine (TJP) gebildet.
Netrin-1 (Ntn1) hat je nach Rezeptorbindung verschiedene Effekte auf TJP und somit auf
die Barriereeigenschaften.
In dieser Arbeit wurde im Tiermodell (Chronic Constriction Injury-CCI) untersucht, ob
Netrin-1 einen Einfluss auf die BNS hat und die Wirkung der Rezeptoren Unc5b und
Neogenin-1 beleuchtet. Es wurde untersucht, ob der barrierestabilisierende Netrin-1-
Spiegel auch von neuropathischen Schmerzen, im Speziellen durch „Chronic Regional
Pain Syndrom“ (CRPS), beeinflusst wird.
Männl. Wistar-Ratten wurde lokal Unc5b Antikörper injeziert oder nach Netrin-1 Gabe
der Neogeninrezeptor durch lokale Neogenin-1-siRNA Injektion geblockt. Die mRNA
Expression von Ntn1, seine Rezeptoren sowie der TJP (Claudine-Cldn) wurde mittels q-
PCR untersucht. Netrin-1 wurde im Rattennerven mittels Western Blot bestimmt. Die
Netrin-1-Spiegel im Plasma von CRPS Patient*innen und Kontrollen wurde mittels ELISA
bestimmt. Im Rattenmodell war die Ntn1 vermehrt exprimiert, die Proteinexpression
mittels Western Blot tendenziell vermindert. Die Claudinexpression war nach CCI
herabreguliert. Netrin-1-Injektion steigerte die Expression von Cldn5 und 19. Der
Netrin-1-Rezeptor UNC5B wird bei Neuropathie verstärkt und Neogenin-1 vermindert
exprimiert. Die Expression von Cldn 12 und Cldn19 war bei Blockade des Unc5b
Rezeptors gesteigert und bei Blockade des Neogenin-1 Rezeptors tendenziell
vermindert. Im Plasma von CRPS Patient*innen zeigte sich ein verminderter Netrin-1-
Spiegel.
Die Ergebnisse der vorliegenden Experimente legen nahe, dass Netrin-1 über die
Stabilisierung der Blut-Nerven-Schranke einen lindernden Effekt auf neuropathische
Schmerzen hat und sich auch die Expression dieses Proteins durch CRPS verändert.
Fabry Disease (FD) is a genetic lysosomal storage disorder based on mutations in the gene encoding α-Galactosidase A (α-GalA) leading to accumulation of globotriaosylceramide (Gb3). Missense mutations induce an amino acid exchange (AAE) in the α-GalA. Pain is a predominant symptom in FD and the pathophysiology is unclear. Skin punch biopsies were obtained from 40 adult FD patients and ten healthy controls and dermal fibroblast cultures were generated for cell culture experiments to investigate Gb3 load, gene and protein expression patterns and ion channel activity. The 3D-structure of α-GalA was downloaded into Pymol Graphics System and the AAE was depicted and located in order to investigate the correlation between the AAE location type in the α-GalA and the clinical FD phenotype.
FD dermal fibroblasts showed high Gb3 load depending on treatment interval and expressed Kca1.1 channels. Activity was reduced in FD cells at baseline, but increased over-proportionately upon Gb3-cleavage by enzyme replacement therapy. Gene and protein expression of Kca1.1 was increased in FD cells. FD dermal fibroblasts showed higher gene expression of Notch1 and several cytokines. Further, it was shown that three different AAE location types can be differentiated: mutations in the active site (‘active site’), those buried in the core of α-GalA (‘buried’) and those at another location, mostly on the protein surface (‘other’). FD patients carrying active site or buried mutations showed a severe clinical phenotype with multi-organ manifestation and early disease onset. Patients with other mutations were less severely affected with oligo-organ manifestation sparing the nervous system and later disease onset.
These results show that dermal fibroblasts may be involved in FD-associated pain and that stratification of FD patients carrying missense mutations by AAE location type may be an advantageous parameter that can help in the management of FD patients.
CD137 und CD137L stellen ein Rezeptor-Liganden-Paar dar, welches auf vielen Immunzellen exprimiert wird und eine wichtige Rolle im Rahmen der Immunstimulation spielt. CD137L fungiert jedoch nicht nur als Ligand sondern auch als Rezeptor und vermittelt als ein solcher Signale in die ihn exprimierende Zelle. Neben seinem Vorkommen auf Immunzellen wird CD137L auch von einigen Tumorzellen exprimiert, unter anderem auch auf denen des Kolonkarzinoms. In dieser Tumorentität korreliert eine hohe CD137L-Expression mit dem Auftreten von Fernmetastasen und einer insgesamt schlechteren Prognose. Die genaue Rolle von CD137L im Kolonkarzinom ist bislang kaum erforscht. Im Rahmen dieser Arbeit wurden daher die Auswirkungen einer CD137L-Aktivierung auf die Proliferation sowie die Proteinexpression und -sekretion von Kolonkarzinomzellen untersucht.
Die Ergebnisse deuten erstmals darauf hin, dass die CD137L-Stimulation in vitro die Proliferation der entarteten Zellen reduziert und die Expression bzw. Sekretion der Proteine Vimentin, TLR7, VEGF und PDGF steigert.
Hieraus wird geschlossen, dass eine Stimulation des von den Kolonkarzinomzellen exprimierten CD137L dazu führt, dass sich der Phänotyp der Tumorzellen von einem epithelialen in Richtung eines mesenchymalen Zelltyps verändert. Darüber hinaus werden vermehrt Proteine exprimiert und sezerniert, welche über unterschiedliche Signalwege an der Invasion und Migration der entarteten Zellen beteiligt sind.
Folglich lässt sich annehmen, dass CD137L eine entscheidende Rolle im Metastasierungsprozess von humanen Kolonkarzinomzellen spielt. Sollte sich dies in weiterführenden Untersuchungen bestätigen, könnte eine pharmazeutische Beeinflussung der beteiligten Signalwege möglicherweise die Prognose von an Kolonkarzinomen erkrankten Patient:innen deutlich verbessern.
Ziel: Erfassung von Morbidität, hypokalzämischen Ereignissen, subjektiver Lebensqualität (QoL) und Leistungsfähigkeit im täglichen Leben bei Patienten mit chronischem Hypoparathyreoidismus (HPT).
Methoden: Kontaktierung von Patienten mit chronischem HPT und Pseudo-HPT (n=412) aus mehreren deutschen Zentren mit Antwort von 215 (Rücklaufquote 52%) und Einschluss von 205 Probanden. Die Patienten erhielten einen krankheitsspezifischen Fragebogen. Die Bewertung der subjektiven QoL erfolgte mit dem SF-36-Fragebogen. Die Daten wurden mit geschlechts- und altersangepassten Kontrollen verglichen.
Ergebnisse: SF-36-Werte zeigten bei HPT-Patienten im Vergleich zur Allgemeinbevölkerung eine signifikant beeinträchtigte QoL. Die Lebenszeitprävalenz von Depressionen, Angstzuständen und Niereninsuffizienz war bei HPT-Patienten höher (22% vs. 15%; 21% vs. 6%; 11% vs. 2%, alle p<0,01). Signifikant mehr Patienten erhielten Antihypertensiva oder Antiepileptika (44% vs. 34%, p=0,003; 5% vs. 2%, p=0,01). Die Prävalenz von Katarakt, Herzrhythmusstörungen, Nierensteinen und Nephrokalzinose betrug 18%, 16%, 8% bzw. 6%. 37% berichteten über Einschränkungen im Beruf und mehr HPT-Patienten waren im Alter von <65 Jahren berentet verglichen zu Kontrollen (21% vs. 10%, p<0,001). HPT-Patienten gaben häufiger Beeinträchtigungen von Alltagsaktivitäten aus gesundheitlichen Gründen an (58% vs. 32%, p<0,001). Seit der Erstdiagnose waren 36% der Patienten mind. einmal in einer Notaufnahme vorstellig oder wegen HPT ins Krankenhaus eingeliefert worden. Die Gesamthäufigkeit von hypokalzämischen Notfällen mit notwendiger i.v. Calciumgabe betrug 14,7 Ereignisse pro 100 Patientenjahre. Die Hospitalisierungsrate innerhalb der letzten 12 Monate unterschied sich nicht signifikant von der Kontrollgruppe.
Schlussfolgerung: Trotz etablierter Therapie ist ein chronischer HPT mit einer reduzierten subjektiven QoL, einer Vielzahl von Komplikationen und einer Beeinträchtigung des Alltags- und Berufslebens assoziiert.
Das Multiple Myelom (MM) ist eine seltene, maligne Störung der Plasmazellen, welche trotz gehöriger Therapiefortschritte in den letzten Jahrzehnten nach wie vor als unheilbare Erkrankung betrachtet werden muss. Obwohl eine sehr große intra- und interindividuelle Heterogenität beim Multiplen Myelom beobachtet werden kann, gibt es verschiedene Mutationen, die mit höherer Frequenz in Myelompatientinnen und -patienten gefunden werden. Eines dieser häufiger betroffenen Proteine ist KRas mit Mutationen in etwa 20% der Fälle. Da die Ras-Proteine und somit auch ihre Isoform KRas zu Beginn der Ras/Raf/Mek/Erk-Signalkaskade stehen und dementsprechend einen großen Einfluss auf die Übermittlung von Wachstums- und Überlebenssignalen in Zellen besitzen, ist eine nähere funktionelle Analyse verschiedener KRas-Mutationen von großer Relevanz. Während für einige Mutationen von KRas bereits funktionelle Analysen existieren, wurden die häufig auftretende Exon 2-Mutation KRasp.G12A, sowie die beiden seltenen Exon 4-Mutationen KRasp.A146T und KRasp.A146V bisher in ihrer funktionellen Rolle im MM noch nicht näher charakterisiert. Um die funktionellen Aspekte dieser genannten Mutationen von KRas näher zu untersuchen, kamen im Rahmen meiner Versuchsreihe Sleeping Beauty Transposon System basierte Expressionsvektoren zur transienten und dauerhaften Proteinexpression in verschiedenen Myelomzelllinien zum Einsatz. Durch Transfektion dieser Plasmide in die KRas-Wildtyp tragenden Zellen mit nachfolgender Transposition in die genomische DNA konnte gezielt die Überexpression der verschiedenen Mutationen realisiert werden.
So konnte durch die funktionelle Proteinauslese mittels der Anfertigung von Western Blots gezeigt werden, dass jede der drei getesteten Mutationen zu einer verstärkten Phosphorylierung und damit Aktivierung von KRas-nachgeschalteten Proteinen wie z.B. Erk führt. Zusätzlich wurde für die KRas-Mutationen auch ein aktivierender Effekt auf den PI3K/Akt-Signalweg anhand einer erhöhten Phosphorylierung des Proteins Akt nachgewiesen.
Ebenso wie andere bereits besser charakterisierte KRas-Mutationen haben demnach auch die getesteten KRas-Mutationen KRasp.G12A, KRasp.A146T und KRasp.A146V einen positiven Einfluss auf die intrazellulären Überlebenssignale und könnten daher eine elementare Rolle in der Entwicklung des Multiplen Myeloms bei Patientinnen und Patienten spielen. Es gilt daher, die drei in dieser Arbeit untersuchten KRas-Mutationen, zukünftig in die Wirkstoffsuche KRas-spezifischer Therapeutika miteinzubeziehen.
Es war ein wichtiger Meilenstein in der Behandlung der pulmonalarteriellen Hypertonie, als herausgefunden wurde, dass gewisse Patienten mit positivem Vasoreagibilitätstest von einer Langzeittherapie mit Kalziumkanalantagonisten profitieren und eine deutlich erhöhte Lebenserwartung aufweisen. Seit 2018 werden diese Patienten einer eigenen Subklassifikation zugeordnet. Das Augenmerk dieser retrospektiven Studie lag daher insbesondere darauf, welche vasoresponsiven Patienten zur neuen Gruppe der Langzeit-Kalziumkanalblocker-Responder gehören und ob das Konzept der wiederholten Vasoreagibilitätstestungen in diesem Kollektiv umgesetzt wird.
Das Nebennierenkarzinom ist ein sehr selten auftretender Tumor. Mitotane ist derzeit das einzige zugelassene Medikament zur adjuvanten und palliativen Behandlung von betroffenen Patienten. Eine häufig beobachtete Nebenwirkung ist die Veränderung des Schilddrüsenstoffwechsels. Mithilfe des Nebennierenkarzinomregisters ENSAT konnten in die vorliegende Arbeit 129 Patienten eingeschlossen und deren Patientendaten retrospektiv ausgewertet werden. Somit konnte insbesondere die Frage des veränderten Schilddrüsenstoffwechsels in den Patientengruppen ohne Schilddrüsenmedikation und mit Medikation untersucht und ein möglicher Einfluss der Einnahme von Levothyroxin beantwortet werden. Bei der Auswertung der Patientendaten zeigte sich, dass eine isolierte fT4-Erniedrigung in beiden Patientengruppen sehr häufig auftritt (Gruppe ohne Schilddrüsen-Medikamente 60 %; Gruppe mit Schilddrüsenmedikamenten 57 %). Bei Auswertung der fT4-Werte war auffällig, dass Patienten mit einer erniedrigten fT4-Stoffwechsellage unter Mitotanetherapie, schon vor Therapiebeginn signifikant niedrigere Ausgangswerte aufwiesen (Patientengruppe mit Schilddrüsenmedikation 18,3 pmol/l vs. 15,65 pmol/l; Patientengruppe ohne Schilddrüsenmedikation 17,3 pmol/l vs. 14,75 pmol/l). Unter Mitotanetherapie konnte sowohl in den Gruppen mit erniedrigter fT4-Stoffwechsellage, als auch ohne isoliert erniedrigtes fT4 ein Absinken des fT4-Wertes beobachtet werden. Jedoch kam es nicht in jedem Fall zu einer Änderung des Schilddrüsenstoffwechsels mit der Stoffwechsellage „erniedrigtes fT4“. TSH und fT3 zeigten keine signifikante Veränderung in beiden Patientenkollektiven unter Mitotanetherapie. Da der Mitotanespiegel nur mit geringer, bzw. keiner Effektstärke mit dem fT4-Wert korrelierte, kann mithilfe der Höhe der Mitotaneplasmakonzentration keine definitive Vorhersage zur Entwicklung des fT4-Wertes gemacht werden. Mithilfe eines In-vitro-Experimentes konnte widerlegt werden, dass isoliert erniedrigtes fT4 aufgrund eines messtechnischen Fehlers im Labor entsteht. Eine durchgeführte, kontrollierte Levothyroxin-Gabe bei Patienten mit erniedrigter fT4-Stoffwechsellage zeigte, dass bei regelmäßiger Kontrolle der Schilddrüsenparameter und individueller Levothyroxin-Dosisanpassung eine Normalisierung des Schilddrüsenfunktionsstoffwechsels in 71 % erreicht werden konnte. Ein allgemeiner Behandlungsstandard konnte nicht erkannt werden, da der Bedarf an Levothyroxin sehr unterschiedlich war. Eine Anfangsdosis von 50 µg scheint empfehlenswert und sollte anschließend abhängig von den Laborwerten in den Verlaufskontrollen angepasst werden. Nach wie vor ist der Pathomechanismus, welcher zur isolierten fT4-Erniedrigung führt, nicht vollständig geklärt. Am wahrscheinlichsten erscheint eine Induktion von TBG durch Mitotane. Insgesamt zeigt diese Arbeit erstmals in einer großen Kohorte, wenn auch mit eingeschränkter Aussagekraft aufgrund der retrospektiven Erhebung, die Häufigkeit und Art und Weise der Veränderungen von Schilddrüsenfunktionstests unter einer Mitotanetherapie und ermöglicht so bei behandelnden Ärzten eine genauere Einschätzung der Relevanz dieser Nebenwirkung.
Zellstress in Form von lytischer Herpes-simplex-Virustyp-1-Infektion, Hitze und Salzstress führt dazu, dass die RNA-Polymerase II über das 3'-Ende von manchen Genen hinaus transkribiert. Dies geht bei Herpes-simplex-Virustyp-1-Infektion teilweise mit offenem Chromatin nach dem 3'-Ende einher. In dieser Arbeit wurden verschiedene Methoden getestet, um diese Effekte genomweit zu eruieren. Dabei wurden die Peak-Caller ATAC-seq-Pipeline, F-Seq, Hotspots und MACS2 getestet sowie mit der Hilfsgröße „downstream Open Chromatin Regions“ gearbeitet. Weiterhin wurde das R-Skript „Pipeline for ATAC-seq and 4sU-seq plotting“ entwickelt, mit dem sich die Dynamik der oben beschriebenen Effekte zeigen lässt: Die Offenheit des Chromatins ist bei Herpesinfektion zusätzlich zur Erhöhung nach dem 3'-Ende generell erhöht. Die Transkription der RNA-Polymerase II über das 3'-Ende hingegen nimmt nach 75k Basenpaaren rapide ab. Die Ergebnisse des R-Skripts im Bezug auf Salz und Hitzestress decken sich mit vorbeschriebener Literatur, in der gezeigt wurde, dass eine Erhöhung der Offenheit des Chromatins nach dem 3'-Ende nicht stattfindet.
In den letzten Jahren gewann das Konzept der Paranodopathien als eigene Krankheitsentität der inflammatorischen Polyneuropathien zunehmend an Bedeutung. Die Forschung konzentrierte sich dabei überwiegend auf die chronisch inflammatorische Polyradikuloneuropathie (CIDP). In dieser Arbeit werden (para-)nodale Antikörper gegen Neurofascin-155, panNeurofascin, Contactin-1 und Caspr-1 in einer großen Kohorte von Patienten mit Guillain-Barré-Syndrom (GBS) und CIDP nachgewiesen. Patienten mit Anti-panNeurofascin-Antikörpern zeigten besonders schwere Verlaufsformen. Patienten mit anderen (para-)nodalen Antikörpern zeigten je nach IgG-Subklasse der Antikörper spezifische klinische Merkmale und ein unterschiedliches Ansprechen auf die Therapie. Die Arbeit zeigt, dass die Bestimmung (para-)nodaler Antikörper bei Patienten mit GBS und CIDP im klinischen Alltag zur Einordung der Prognose und Therapieplanung sinnvoll sein kann.
Herzinsuffizienz ist eine sehr häufige Erkrankung im hohen Lebensalter mit zudem signifikant hoher Mortalität - vergleichbar mit der Mortalität häufiger Krebsarten. Biomarker wie die natriuretischen Peptide sind von großer Wichtigkeit hinsichtlich der Diagnosestellung und Prognoseabschätzung. Auch inflammatorische Marker, Copeptin sowie Mid-regionales Adrenomedullin (MR-proADM) haben eine wichtige Rolle sowohl in der Diagnosestellung der Herzinsuffizienz als auch in der Prognoseabschätzung eingenommen. Die Aussagekraft der Biomarker in einem diagnostisch naiven Kollektiv mit dem klinisch-anamnestischen Verdacht auf das Vorliegen einer Herzinsuffizienz ist jedoch bisher kaum untersucht worden.
Die Handheld-BNP-Studie schloss diagnostisch naive Patienten ein, die sich mit Symptomen passend zu einer Herzinsuffizienz beim Hausarzt vorstellten. Binnen 14 Tagen erfolgte die Referenzdiagnose durch einen niedergelassenen Kardiologen. Ziel war es, die diagnostische Aussagekraft von BNP und der miniaturisierten Echokardiographie im primärärztlichen Bereich zu überprüfen. Die vorliegenden Follow-Up-II-Untersuchung untersuchte die prognostische Aussagekraft moderner Biomarker (N-terminales B-natriuretisches Peptid (NT-proBNP), Mid-regionales atriales natriuretisches Peptid (MR-proANP), Mid-regionales Adrenomedullin (MR-proADM), Copeptin, Tumornekrosefaktor Alpha (TNF- α) und hochsensitives C-reaktives Protein (hsCRP)). Die Endpunkte waren Tod jeder Ursache sowie kardiovaskulärer Tod.
Insgesamt traten in unseren Analysen die natriuretischen Peptide mit ihrer prognostischen Aussagekraft hervor. In den univariaten Analysen zeigte sich das NT-proBNP als wichtigster Biomarker und in den multivariaten Analysen das MR-proANP.
Bei diagnostisch naiven Patienten, die sich mit Herzinsuffizienzsymptomen bei ihrem Hausarzt vorstellen, besteht ein hohes Mortalitätsrisiko. Um diese Patienten adäquat zu selektieren, eine leitliniengerechte Therapie einzuleiten und um das Fortschreiten der Erkrankung aufzuhalten, ist eine frühzeitige Diagnosestellung beim Kardiologen wichtig. Natriuretische Peptide sind prädiktiv, jedoch stellt das MR-proANP aufgrund fehlender generalisierter Verfügbarkeit keine realistische Option im primärärztlichen Bereich dar. Das NT-proBNP hat eine flächendeckende Verfügbarkeit und wird mittlerweile in den Herzinsuffizienz-Leitlinien der ESC bei der Verdachtsdiagnose Herzinsuffizienz standardmäßig empfohlen.
Wilms tumor (WT) is the most common renal tumor in childhood. Among others, MYCN copy number gain and MYCN P44L and MAX R60Q mutations have been identified in WT. The proto-oncogene MYCN encodes a transcription factor that requires dimerization with MAX to activate transcription of numerous target genes. MYCN gain has been associated with adverse prognosis. The MYCN P44L and MAX R60Q mutations, located in either the transactivating or basic helix-loop-helix domain, respectively, are predicted to be damaging by different pathogenicity prediction tools. These mutations have been reported in several other cancers and remain to be functionally characterized.
In order to further describe these events in WT, we screened both mutations in a large cohort of unselected WT patients, to check for an association of the mutation status with certain histological or clinical features. MYCN P44L and MAX R60Q revealed frequencies of 3 % and 0.9 % and also were significantly associated to higher risk of relapse and metastasis, respectively. Furthermore, to get a better understanding of the MAX mutational landscape in WT, over 100 WT cases were analyzed by Sanger sequencing to identify other eventual MAX alterations in its coding sequence. R60Q remained the only MAX CDS alteration described in WT to date.
To analyze the potential functional consequences of these mutations, we used a doxycycline-inducible system to overexpress each mutant in HEK293 cells. This biochemical characterization identified a reduced transcriptional activation potential for MAX R60Q, while the MYCN P44L mutation did not change activation potential or protein stability. The protein interactome of N-MYC-P44L was likewise not altered as shown by mass spectrometric analyses of purified N-MYC complexes. However, we could identify a number of novel N-MYC partner proteins, several of these known for their oncogenic potential. Their correlated expression in WT samples suggested a role in WT oncogenesis and they expand the range of potential biomarkers for WT stratification and targeting, especially for high-risk WT.
D-4F, ein ApoA-I-mimetisches Peptid, lindert mechanische Hyperalgesie nach Verabreichung an Nagetiere, die an entzündlichen und neuropathischen Schmerzen leiden. D-4F fängt proalgetische oxidierte Lipide ab – als ein solches entzündungshemmendes Medikament – aktiviert aber auch ATP-bindende Kassettentransporter (Abca1 und Abcg1) – als ein solches anti-atherosklerotisches Medikament. Ziel dieser Arbeit war es, den Einfluss von Neuropathie und deren Behandlung mit D-4F auf die Expression von Abca1/Abcg1 sowie Zytokinen zu untersuchen.
172 Patienten, welche von 06/17 bis 01/18 am Uniklinikum Würzburg entsprechend der Ross-Prozedur operiert wurden, wurden nachuntersucht (Follow Up: 6,2 ± 4,5 Jahre).
Ziel war es den Einfluss der Operationstechniken - Subkoronar (SK)- und Wurzelersatztechnik (WE) zu untersuchen sowie den Einfluss der Spenderklappen - Homograft (HG) und Matrixprothese (M).
Die Überlebensrate betrug in der SK- und WE-Gruppe jew. 98,1 %. Die Reoperationsrate lag bei 6,8 % (SK) und 3,9 % (WE). Bezüglich der postop. Hämodynamik wurden Normwerte ermittelt (SK und WE). NYHA Stadium I lag bei 74,1 % (SK) und 95,5 % (WE) vor.
Bzgl. Überlebensrate (HG: 98,1 %; M: 95,4 %) sowie Reop.rationsrate (HG: 0 %; M: 33 %), postop. hämodyn. Werten und postop. klinischem Verlauf (NYHA Stadium I: 80 % der HG Gruppe, 50 % der M-Gruppe) schnitt die M-Gruppe schlechter ab.
Das Nebennierenrindenkarzinom (ACC) ist eine sehr seltene maligne Erkrankung, die mit einer infausten Prognose vergesellschaftet ist. In Zeiten apparativ geprägter Medizin treten suspekte Befunde der Nebenniere gehäufter auf als je zuvor. Diese Nebennierenraumforderungen, die zumeist bei Bildgebungen auffallen, die aus anderen Gründen indiziert waren, werden Nebenniereninzidentalome genannt und sind meist benigne Befunde. Dennoch wird es angesichts dieser steigenden Zahl an Inzidentalomen immer wichtiger, die Entität der gefundenen Raumforderung schnell zu sichern, um die entsprechende Therapie einleiten zu können. Somit sollen das Zeitfenster bis zur Krebstherapie verkleinert und gleichsam unnötige chirurgische Eingriffe bei Patient*innen mit benignen Nebennierentumoren vermieden werden. Um die diagnostischen Schritte weiter zu verbessern, wurde in der vorliegenden Arbeit eine bioinformatische Regressionsanalyse an Steroidhormonkonzentrationen von ACC-Patient*innen und Kontrollen durchgeführt und der diagnostische Wert der berechneten Steroidsignaturen untersucht. Dabei zeigte sich im geschlechtsspezifischen Modell jeweils eine 6-Steroid-Signatur mit bester Trennschärfe zwischen benignen und malignen NN-Befunden. So konnte mit der 6-Steroid-Signatur in der männlichen Patientengruppe mit einer Sensitivität von 80% und Spezifität von 97%, in der weiblichen Patientinnengruppe mit einer Sensitivität von 78% und Spezifität von 97% die Diagnose richtig zugewiesen werden. Im Rahmen der targeted Metabolomics Untersuchung konnten Tumor-assoziierte Stoffwechselalterationen aufgezeigt werden. Eine Plasma-Metabolit-Signatur zur Differenzierung von ACCs und Nebennierenadenomen, welche die gängige Diagnostik bei der Abklärung von unklaren Nebennierentumoren erleichtern könnte, erscheint jedoch angesichts der großen Anzahl an zu bestimmenden Metaboliten - auch unter ökonomischen Gesichtspunkten - zu diesem Zeitpunkt noch nicht mit der Routine-Patient*innenversorgung vereinbar.
Wilms tumor (WT) or nephroblastoma is the most common kidney tumor in childhood. Several genetic alterations have been identified in WT over the past years. However, a clear-cut underlying genetic defect has remained elusive. Growing evidence suggests that miRNA processing genes play a major role in the formation of pediatric tumors, including WT.
We and others have identified the microprocessor genes DROSHA and DGCR8 as key players in Wilms tumorigenesis. Exome sequence analysis of a cohort of blastemal-type WTs revealed the recurrent hotspot mutations DROSHA E1147K and DGCR8 E518K mapping to regions important for catalyic activity and RNA-binding. These alterations were expected to affect processing of miRNA precursors, ultimately leading to altered miRNA expression. Indeed, mutated tumor samples were characterized by distinct miRNA patterns. Notably, these mutations have been observed almost exclusively in WT, suggesting that they play a specific role in WT formation.
The aim of the present work was to first examine the mutation frequency of DROSHA E1147K and DGCR8 E518K in a larger cohort of WTs, and to further characterize these microprocessor gene mutations as potential oncogenic drivers for WT formation.
Screening of additional 700 WT samples by allele-specific PCR revealed a high frequency of DROSHA E1147K and DGCR8 E518K mutations, with the highest incidence found in tumors of high-risk histology. DROSHA E1147K was heterozygously expressed in all cases, which strongly implies a dominant negative effect. In contrast, DGCR8 E518K exclusively exhibited homozygous expression, suggestive for the mutation to act recessive.
To functionally assess the mutations of the microprocessor complex in vitro, I generated stable HEK293T cell lines with inducible overexpression of DROSHA E1147K, and stable mouse embryonic stem cell (mESC) lines with inducible overexpression of DGCR8 E518K. To mimic the homozygous expression observed in WT, DGCR8 mESC lines were generated on a DGCR8 knockout background. Inducible overexpression of wild-type or mutant DROSHA in HEK293T cells showed that DROSHA E1147K leads to a global downregulation of miRNA expression. It has previously been shown that the knockout of DGCR8 in mESCs also results in a significant downregulation of canonical miRNAs. Inducible overexpression of wild type DGCR8 rescued this processing defect. DGCR8 E518K on the other hand, only led to a partial rescue. Differentially expressed miRNAs comprised members of the ESC cell cycle (ESCC) and let-7 miRNA families whose antagonism is known to play a pivotal role in the regulation of stem cell properties. Along with altered miRNA expression, DGCR8-E518K mESCs exhibited alterations in target gene expression potentially affecting various biological processes.
We could observe decreased proliferation rates, most likely due to reduced cell viability. DGCR8-E518K seemed to be able to overcome the block of G1-S transition and to rescue the cell cycle defect in DGCR8-KO mESCs, albeit not to the full extent like DGCR8-wild-type. Moreover, DGCR8-E518K appeared to be unable to completely block epithelial-to-mesenchymal transition (EMT). Embryoid bodies (EBs) with the E518K mutation, however, were still able to silence the self-renewal program rescuing the differentiation defect in DGCR8-KO mESCs.
Taken together, I could show that DROSHA E1147K and DGCR8 E518K are frequent events in WT with the highest incidence in high-risk tumor entities. Either mutation led to altered miRNA expression in vitro confirming our previous findings in tumor samples. While the DROSHA E1147K mutation resulted in a global downregulation of canonical miRNAs, DGCR8 E518K was able to retain significant activity of the microprocessor complex, suggesting that partial reduction of activity or altered specificity may be critical in Wilms tumorigenesis.
Despite the significant differences found in the miRNA and mRNA profiles of DGCR8 E518K and DGCR8-wild-type mESCs, functional analysis showed that DGCR8 E518K could mostly restore important cellular functions in the knockout and only slightly differed from the wild-type situation. Further studies in a rather physiological environment, such as in a WT blastemal model system, may additionally help to better assess the subtle differences between DGCR8 E518K and DGCR8 wild-type observed in our mESC lines. Together with our findings, these model systems may thus contribute to better understand the role of these microprocessor mutations in the formation of WT.
Bereits vorangegangene Studien haben zeigen können, dass eine verstärke Generali- sierung von Furcht sowohl bei Erwachsenen, bei denen beispielsweise eine Angststörung oder eine PTSB diagnostiziert wurde, aber auch bei gesunden Kindern eine Rolle spielt. In unserer Studie untersuchten wir eine Gruppe Kinder und Jugendliche (n = 31, m = 25, w = 6; Alter = 13.35 ± 2.03), die eine Störung des Sozialverhaltens aufwiesen, auf die Konditionierbarkeit von Furcht und eine mögliche Furchtgeneralisierung. Diese Gruppe verglichen wir mit einer gesunden Kontrollgruppe (n = 29, m = 11, w = 18; Alter = 14.28 ± 2.43). Als Generalisierungsstimuli verwendeten wir ein Furchtgeneralisierungsparadigma mit zwei Frauengesichtern, die in vier Schritten aneinander angeglichen wurden. Zusätzlich führten wir mit beiden Probandengruppen ein Dot-Probe-Paradigma zur Objektivierung von Aufmerksamkeitsprozessen im Sinne eines Attentional Bias oder Attentional Avoidance mit emotionalen Gesichtern durch. Wir konnten eine erfolgreiche Furchtkonditionierung für beide Gruppen erreichen. Im Vergleich mit der gesunden Kontrollgruppe zeigte die externalisierende Probandengruppe eine verstärke Furchtgeneralisierung.
Hinsichtlich der subjektiven Valenz- und Kontingenzratings wurden die Unterschiede besonders deutlich. Eine verstärkte Generalisierungsneigung bei erhöhter Trait-Angst konnten wir nicht finden. Die externalisierende Gruppe zeigte im Vergleich mit neutralen Gesichtern bei den emotionalen Gesichtern insgesamt einen Attentional Bias. Am deutlichsten war dabei eine verstärkte Aufmerksamkeitslenkung hin zu glücklichen Gesichtern festzustellen. Für die gesunde Kontrollgruppe konnten wir keine Besonderheiten bezüglich der Aufmerksamkeitsrichtung finden. Weiterführende Studien sollten mit größ- eren Probandengruppen und nach Geschlecht und Alter gepaarten Probanden durch- geführt werden. Mit externalisierenden Probanden sollte ein Furchtgeneralisierungs- paradigma mit neutralen Stimuli (z.B. Ringe) gewählt werden, um eine subjektive Wertung emotionaler Gesichter bei den Ratings als Störfaktor auszuschließen. Für externalisierende Probanden sollte außerdem die Ausprägung von CU-Traits erfasst und die Dauer der Testung verkürzt oder auf zwei Termine aufgeteilt werden, um eine ausreichende Konzentrationsfähigkeit zu ermöglichen.
