A mouse model for genetic deletion of presynaptic BDNF from adult hippocampal mossy fiber terminals

Brain-derived neurotrophic factor (BDNF) is a modulator and mediator of structural and functional plasticity at synapses in the central nervous system. Despite our profound knowledge about the synaptic function of BDNF at synapses, it is still controversially discussed whether synaptic BDNF acts primarily from pre- or postsynaptic sites. In the central nervous system, several studies show that mossy fiber (MF) projections formed by hippocampal granule neurons store the highest amount of BDNF. However, immunofluorescence and RNA labelling studies suggest that MF BDNF is primarily produced by granule neurons. Multiple other studies prefer the view that BDNF is primarily produced by postsynaptic neurons such as CA3 pyramidal neurons. Here, we question whether the BDNF, which is stored in the mossy fiber synapse, is primarily produced by granule neurons or whether by other cells in the MF-CA3 microcircuit. After standardization of immunolabelling of BDNF, confocal imaging confirmed the localization of BDNF in presynaptic MF terminals. This anterograde location of synaptic BDNF was also found in distinct regions of the fear and anxiety circuit, namely in the oval nucleus of the bed nucleus stria terminals (ovBNST) and in the central amygdala. To find out whether the presynaptic BDNF location is due to protein translation in the corresponding presynaptic dentate gyrus (DG) granule neuron, we developed and characterized a mouse model that exhibits BDNF deletion specifically from adult DG granule neurons. In this mouse model, loss of presynaptic BDNF immunoreactivity correlated with the specific Creactivity in granule neurons, thus confirming that MF BDNF is principally released by granule neurons. After BDNF deletion from granule neurons, we observed more immature neurons with widely arborized dendritic trees. This indicated that local BDNF deletion also affects the local adult neurogenesis, albeit Cre-mediated BDNF deletion only occur in adult granule neurons. Since BDNF is a master regulator of structural synaptic plasticity, it was questioned whether it is possible to visualize presynaptic, synapse-specific, structural plasticity in mossy fiber synapses. It was established that a combination of Cre-techniques together with targeting of GFP to membranes with the help of palmitoylation / myristoylation anchors was able to distinctly outline the synaptic structure of the BDNF-containing MF synapse. In summary, the mouse model characterized in here is suited to investigate the synaptic signalling function of presynaptic BDNF at the mossy fiber terminal, a model synapse to investigate microcircuit information processing from molecule to behaviour.
Background
Chronic thromboembolic pulmonary hypertension (CTEPH) is a long-term complication following an acute pulmonary embolism (PE). It is frequently diagnosed at advanced stages which is concerning as delayed treatment has important implications for favourable clinical outcome. Performing a follow-up examination of patients diagnosed with acute PE regardless of persisting symptoms and using all available technical procedures would be both cost-intensive and possibly ineffective. Focusing diagnostic procedures therefore on only symptomatic patients may be a practical approach for detecting relevant CTEPH.
This study aimed to evaluate if a follow-up program for patients with acute PE based on telephone monitoring of symptoms and further examination of only symptomatic patients could detect CTEPH. In addition, we investigated the role of cardiopulmonary exercise testing (CPET) as a diagnostic tool.
Methods
In a prospective cohort study all consecutive patients with newly diagnosed PE (n=170, 76 males, 94 females within 26 months) were recruited according to the inclusion and exclusion criteria. Patients were contacted via telephone and asked to answer standardized questions relating to symptoms. At the time of the final analysis 130 patients had been contacted. Symptomatic patients underwent a structured evaluation with echocardiography, CPET and complete work-up for CTEPH.
Results
37.7%, 25.5% and 29.3% of the patients reported symptoms after three, six, and twelve months respectively. Subsequent clinical evaluation of these symptomatic patients saw 20.4%, 11.5% and 18.8% of patients at the respective three, six and twelve months time points having an echocardiography suggesting pulmonary hypertension (PH). CTEPH with pathological imaging and a mean pulmonary artery pressure (mPAP) ≥ 25 mm Hg at rest was confirmed in eight subjects. Three subjects with mismatch perfusion defects showed an exercise induced increase of PAP without increasing pulmonary artery occlusion pressure (PAOP). Two subjects with pulmonary hypertension at rest and one with an exercise induced increase of mPAP with normal PAOP showed perfusion defects without echocardiographic signs of PH but a suspicious CPET.
Conclusion
A follow-up program based on telephone monitoring of symptoms and further structured evaluation of symptomatic subjects can detect patients with CTEPH. CPET may serve as a complementary diagnostic tool.
Fragestellung
Die Prognose eines akuten Hirninfarktes bei Verschluss einer proximalen Hirnarterie ist trotz der intravenösen Thrombolyse mit rtPA ungünstig. Kann die kombinierte pharmaco-mechanische Rekanalisation von proximalen Gefäßverschlüssen bei akutem Hirninfarkt zu einer Verbesserung des klinischen Ergebnisses führen?
Methoden
Wir analysierten retrospektiv 66 konsekutiv aufgenommene Patienten (36m, 30w; mittleres Alter 61 Jahre (23-86 Jahre), die von 2010 bis 2012 kombiniert pharmako-mechanisch intra-arteriell behandelt wurden. 32 Patienten wiesen einen kombinierten ACI-/M1-Verschluss, 23 einen M1-Verschluss und 11 eine Basilaristhrombose auf. Mittlerer NIHSS lag bei 23. 57 Patienten erhielten eine kombinierte pharmaco-mechanische Therapie, 3 Patienten wurden lediglich pharmakologisch und 6 Patienten rein mechanisch rekanalisiert. Rekanalisierung bei 35 Patienten mit einem Stent-Retriever (32 Patienten mit pREset, 3 Patienten mit SOLITAIRE) erfolgt. Bei 46 Patienten wurde rtPA und bei 32 Patienten Tirofiban als Bridging Verfahren eingesetzt. Eine Stentanlage erfolgte in 28,78% der Fälle.
Ergebnisse
Die erzielten Rekanalisationsraten lagen bei 89,4% bei einer mittleren Dauer der Intervention von 96 Minuten (53,03% unter 90 Min.). Ein günstiges klinisches Ergebnis nach mRS (mRS 0-2) wurde bei 48% der Patienten erreicht. Die Rate an symptomatischen intrazerebralen Blutungen lag bei 4,55%. Die Mortalität war 19,7%. Die multivariate Regressionsanalyse ergab als modifizierbare Prediktoren für ein günstiges Outcome die Dauer bis zur Rekanalisation und die Gabe von rtPA.
Schlussfolgerungen
Die kombinierte endovaskuläre pharmako-mechanische Therapie kann die Mortalität und Morbidität von Schlaganfallpatienten mit Verschlüssen einer proximalen Hirnarterie reduzieren.
Einleitung: Medizinische Trainingsfälle sind in der studentischen Ausbildung inzwischen weit verbreitet. In den meisten Publikationen wird über die Entwicklung und die Erfahrungen in einem Kurs mit Trainingsfällen berichtet. In diesem Beitrag vergleichen wir die Akzeptanz von verschiedenen Trainingsfallkursen, die als Ergänzung zu zahlreichen Vorlesungen der Medizinischen Fakultät der Universität Würzburg mit sehr unterschiedlichen Nutzungsraten eingesetzt wurden, über einen Zeitraum von drei Semestern.
Methoden: Die Trainingsfälle wurden mit dem Autoren- und Ablaufsystem CaseTrain erstellt und über die Moodle-basierte Würzburger Lernplattform WueCampus den Studierenden verfügbar gemacht. Dabei wurden umfangreiche Daten über die Nutzung und Akzeptanz erhoben.
Ergebnisse: Im Zeitraum vom WS 08/09 bis zum WS 09/10 waren 19 Kurse mit insgesamt ca. 200 Fällen für die Studierenden verfügbar, die pro Semester von ca. 550 verschiedenen Medizinstudenten der Universität Würzburg und weiteren 50 Studierenden anderer bayerischer Universitäten genutzt wurden. Insgesamt wurden pro Semester ca. 12000 Mal Trainingsfälle vollständig durchgespielt zu denen ca. 2000 Evaluationen von den Studierenden ausgefüllt wurden. In den verschiedenen Kursen variiert die Nutzung zwischen unter 50 Bearbeitungen in wenig frequentierten Fallsammlungen und über 5000 Bearbeitungen in stark frequentierten Fallsammlungen.
Diskussion: Auch wenn Studierende wünschen, dass zu allen Vorlesungen Trainingsfälle angeboten werden, zeigen die Daten, dass der Umfang der Nutzung nicht primär von der Qualität der verfügbaren Trainingsfälle abhängt. Dagegen werden die Trainingsfälle in fast allen Fallsammlungen kurz vor den Klausuren extrem häufig bearbeitet. Dies zeigt, dass die Nutzung von Trainingsfällen im Wesentlichen von der wahrgenommenen Klausurrelevanz der Fälle abhängt.
Background
Fabry disease (FD) is an X-linked multisystemic disorder with a heterogeneous phenotype. Especially atypical or late-onset type 2 phenotypes present a therapeutical dilemma.
Methods
To determine the clinical impact of the alpha-Galactosidase A (GLA) p.A143T/ c.427G > A variation, we retrospectively analyzed 25 p.A143T patients in comparison to 58 FD patients with other missense mutations.
Results
p.A143T patients suffering from stroke/ transient ischemic attacks had slightly decreased residual GLA activities, and/or increased lyso-Gb3 levels, suspecting FD. However, most male p.A143T patients presented with significant residual GLA activity (~50 % of reference), which was associated with normal lyso-Gb3 levels. Additionally, p.A143T patients showed less severe FD-typical symptoms and absent FD-typical renal and cardiac involvement in comparison to FD patients with other missense mutations. Two tested female p.A143T patients with stroke/TIA did not show skewed X chromosome inactivation. No accumulation of neurologic events in family members of p.A143T patients with stroke/transient ischemic attacks was observed.
Conclusions
We conclude that GLA p.A143T seems to be most likely a neutral variant or a possible modifier instead of a disease-causing mutation. Therefore, we suggest that p.A143T patients with stroke/transient ischemic attacks of unknown etiology should be further evaluated, since the diagnosis of FD is not probable and subsequent ERT or chaperone treatment should not be an unreflected option.
Alzheimer’s disease (AD) is the most prevalent neurodegenerative disease of the brain, which is characterized by a progressive loss of memory and spatial orientation. Only less than 5-10% of AD sufferers are familial cases due to genetic mutations in the amyloid precursor protein (APP) gene or presenilin (PS) 1 and 2 genes. The cause of sporadic AD (sAD) which covers > 95% of AD patients is still unknown. Current research found interactions between aging, diabetes and cognitive decline including dementia in general and in AD in particular. Disturbances of brain glucose uptake, glucose tolerance and utilization and impairment of the insulin/insulin receptor (IR) signaling cascade are thought to be key targets for the development of sAD.
In the brain of AD patients, neural plasticity is impaired indicated by synaptic and neuronal loss. Adult neurogenesis (AN), the generation of functional neurons in the adult brain, may be able to restore neurological function deficits through the integration of newborn neurons into existing neural networks. The dentate gyrus of the hippocampus is one out of few brain regions where life-long AN exists. However, there is a big controversy in literature regarding the involvement of AN in AD pathology. Most animal studies used transgenic mice based on the Amyloid ß (Aß) hypothesis which primarily act as models for the familial form of AD. Findings from human post mortem AN studies were also inconstistent. In this thesis, we focused on the possible involvement of AN in the pathogenesis of the sporadic form of AD. Streptozotocin intracerebroventricularily (STZ icv) treated rats, which develop an insulin-resistant brain state and learning and memory deficits preceding Aß pathology act as an appropriate animal model for sAD. We used STZ treatment for both parts of my work, for the in vivo and in vitro study.
In the first part of my thesis, my coworkers and I investigated STZ icv treatment effects on different stages of AN in an in vivo approach. Even if STZ icv treatment does not seem to considerably influence stem cell proliferation over a short-term (1 month after STZ icv treatment) as well as in a long-term (3 months after STZ icv treatment) period, it results in significantly less immature and newborn mature neurons 3 months after STZ icv treatment. This reduction detected after 3 months was specific for the septal hippocampus, discussed to be important for spatial learning. Subsequently we performed co-localization studies with antibodies detecting BrdU (applied appr. 27 days before sacrifice) and cell-type specific markers such as NeuN, and GFAP, we found that STZ treatment does not affect the differentiation fate of newly generated cells. Phenotype analysis of BrdU-positive cells in the hilus and molecular layer revealed that some of the BrdU-positive cells are newborn oligodendrocytes but not newborn microglia.
In the second part of my thesis I worked with cultured neural stem cells (NSCs) isolated from the adult rat hippocampus to reveal STZ effects on the proliferation of of NSCs, and on the survival and differentiation of their progeny. Furthermore, this in vitro approach enabled me to study cellular mechanisms underlying the observed impaired neurogenesis in the hippocampus of STZ-treated rats. In contrast to our findings of the STZ icv in vivo study we revealed that STZ supplied with the cell culture medium inhibits the proliferation of NSCs in a dose-dependent and time-dependent manner. Moreover, performing immunofluorescence studies with antibodies detecting cell-type specific markers after triggering NSCs to differentiate, we could show that STZ treatment affects the number of newly generated neurons but not of astrocytes. Analyzing newborn cells starting to differentiate and migrate I was able to demonstrate that STZ has no effect on the migration of newborn cells. Trying to reveal cellular mechanisms underlying the negative influence of STZ on hippocampal AN, we performed qRT-PCR and immunofluorescence staining and thus could show that in NSCs the expression of glucose transporter (GLUT)3 mRNA as well as IR and GLUT3 protein levels are reduced after STZ treatment. Therefore, the inhibition of the proliferation of NSCs may be (at least partially) caused by these two molecules. Interestingly, the effect of STZ on differentiating cells was shown to be different, as IR protein expression was not significantly changed but GLUT3 protein levels were decreased in consequence of STZ treatment.
In summary, this project delivered further insights into the interrelation between AN the sporadic form of sAD and thus provides a basis of new therapeutic approaches in sAD treatment through intervening AN. Discrepancies between the results of the two parts of my thesis, the in vivo and in vitro part, were certainly caused to a certain extent by the missing microenvironment in the in vitro approach with cultured NSCs. Future studies e.g. using co-culture systems could at least minimize the effect of a missing natural microenvironment of cultured NSCs, so that the use of an in vitro approach for the investigation of STZ treatment underlying cellular mechanisms can be improved.
Aufgrund der Beeinträchtigung eines oder beider Beine sind viele Menschen zur selbstständigen Fortbewegung im Alltag auf ein Hilfsmittel angewiesen. Gehstützen sind das aufgrund hoher Nutzerfreundlichkeit am häufigsten eingesetzte Hilfsmittel bei kurz oder langandauernder Funktionseinschränkung.
Die im Vergleich zum normalen Gang erhöhte Belastung der oberen Extremitäten kann in schwerwiegenden Komplikationen wie Thrombosen, Gewebe- und Nervenschäden resultieren. Um diese möglichst gering zu halten, wurden im Laufe der Zeit verschiedene Gehstützenmodelle entwickelt.
Ziel der durchgeführten Studie war es, die Belastung der Hände bei verschiedenen hier zu Lande häufig genutzten Gehstützenmodellen und verschiedenen Entlastungen zu erfassen und diese miteinander zu vergleichen. Hierfür zogen wir 26 gesunde Probanden heran, die mit ergonomischen UAG, Arthritis-Gehstützen und anatomischen UAG eine definierte Strecke in festgesetzter, für alle gleicher Reihenfolge liefen. Die zwischen der Hand und dem Gehstützengriff angebrachten Sensormatten erfassten im ersten Studienteil die auf die komplette Hand einwirkende Last und im zweiten Studienteil die Belastung der radialen und ulnaren Hohlhandhälfte separat. Sensorsohlen erfassten zeitgleich die Bodenreaktionskraft.
Die Studie zeigte, dass die mit bis zu 70% des Körpergewichts (KG) beim Gehen mit UAG stark belastete Hand mit Arthritis-Gehstützen deutlich entlastet werden kann. Die Belastung konnte auf 16% verringert werden. Eine schwierigere Handhabe muss jedoch in Kauf genommen werden.
Bei Beschwerden der Hand, besonders im ulnaren Bereich, sollten anatomische UAG gegenüber ergonomischen UAG bevorzugt werden. Die Belastung der ulnaren Handfläche lag hiermit mit 17% des KG deutlich unter der Belastung mit ergonomischen UAG mit 23% des KG. Die Belastung der radialen Hälfte unterschied sich nicht.
Da die Belastung der Hand mit steigender Beinentlastung stieg, sollte die Entlastung möglichst gering und zeitlich kurzgehalten werden.
Der Lagerungsplagiocephalus (sekundärer Plagiocephalus) ist in seiner ansteigenden Inzidenz seit Anfang der Neunziger Jahre ein von verschiedenen Autoren beschriebenes Phänomen. Zunehmend verzeichnet wurde er nach der von dem US Public Health Service im Jahre 1992 veröffentlichten "Back to Sleep Campaign". Zahlreiche kinderärztliche Vereinigungen (wie bspw. die „American Academy of Pedriatrics (APP)“) empfahlen ab diesem Zeitpunkt eine Rückenschlafposition für Neugeborene als SIDS-Prophylaxe (Sudden Infant
Death Syndrome). Der sekundäre Plagiocephalus ist eine Form der Schädeldeformität, dessen Ätiologie auf externe Einflüsse zurückzuführen ist. Durch seinen ähnlichen Phänotypus zur Differentialdiagnose der Craniosynostosen (primäre Plagiocephali) müssen diagnostisch frühzeitig
Verwechslungen ausgeschlossen werden, da sich die therapeutischen Maßnahmen grundlegend unterscheiden. In Fällen schwerer lagerungsbedingter Schädelasymmetrien besteht neben manuellen
Therapieformen die Möglichkeit einer Kopforthesentherapie, die der funktionellen Wachstumssteuerung des Neurokraniums dient.
Aufgrund der aktuellen Diskussion über die Wirksamkeit und Effektivität der
Kopforthesentherapie, der Unstimmigkeit der Krankenkassen zur Kostenübernahme und des großen Interesses der Mitarbeiter bezüglich der Qualität ihrer Arbeit entstand eine Analyse der subjektiven Beurteilung der Beratung, der Betreuung und des Behandlungsprozesses. Auch das Produkt (der Kopforthese) und das Behandlungsergebnis dieser Therapieform wurde durch die Eltern von 83 mit Lagerungsplagiocephalus behandelter Kinder am Cranio Facialen Centrum des Universitätsklinikums Würzburg (CFCW) beurteilt. Des Weiteren sollten die Gründe der Motivation zur Therapieaufnahme und Faktoren einer Reduktion der Besorgnis bzw. Steigerung der Zufriedenheit der Eltern analysiert werden.
Als Erhebungsinstrument dieser Studie diente ein modifizierter Elternfragebogen zur Beurteilung der Behandlung ihres Kindes (FBBm). Die Fragebögen wurden an die Eltern von 33 weiblichen und 60 männlichen Lagerungspagiocephalus-Patienten versandt, die eine abgeschlossene
Kopforthesentherapie zwischen den Jahren 2010 und Januar 2012 verzeichneten.
Die Rücklaufquote der Fragebögen betrug 89,25%. Das Ergebnis der Analyse, zur Motivation der Eltern ein Spezialzentrum aufzusuchen, zeigte, dass diese bei einer erheblichen Anzahl der befragten Eltern aus ihrer seelischen Belastung entsprang, welche sich in Form von Selbstvorwürfen, Ängsten und Sorgen aufgrund der Schädeldeformität ihres Kindes äußerte. Das Ergebnis der Analyse zur Prozessqualität zeigte diesbezüglich eine exzellente Bewertung der befragten Eltern. Die Mitarbeiter des CFCW wurden in der Ausführung ihrer Kommunikationsmethodik
(Gesprächsgestaltung), ihrer Handlungsabläufe und ihrer Betreuung (Hilfestellung) durchweg positiv bewertet. Das Ergebnis der Analyse zur Produktqualität zeigte eine sehr große Akzeptanz und Toleranz im Gebrauch des Therapiemittels. Die Kopforthese wurde in ihrer Wirksamkeit positiv
bewertet, entsprach jedoch nicht vollständig den Qualitätsanforderungen der befragten Eltern. Das äußerst beeindruckende Ergebnis der Analyse zur Ergebnisqualität zeigte eine 96,3%ige Zufriedenheit der Eltern mit dem Behandlungsergebnis ihres Kindes durch Kopforthesentherapie. 96,4% der Befragten waren der Meinung, dass die Helmtherapie für ihr Kind hilfreich war
und sich die Kopfform zudem deutlich besserte.
Weitere Studien zur Zufriedenheit der Eltern mit einem geeigneten Produkt, dessen Defizite behoben wurden und welches den Ansprüchen und Erwartungen der Eltern gerecht wird, sollten zukünftig erfolgen. Eine frühere und umfangreichere Aufklärung der Therapievarianten, insbesondere der Kopforthesentherapie, in der Bevölkerung erscheint sinnvoll, um Eltern und Krankenkassen ihre Verunsicherung bezüglich einer Therapiezustimmung zu nehmen.
Introduction
Genes involved in body weight regulation that were previously investigated in genome-wide association studies (GWAS) and in animal models were target-enriched followed by massive parallel next generation sequencing.
Methods
We enriched and re-sequenced continuous genomic regions comprising FTO, MC4R, TMEM18, SDCCAG8, TKNS, MSRA and TBC1D1 in a screening sample of 196 extremely obese children and adolescents with age and sex specific body mass index (BMI) ≥ 99th percentile and 176 lean adults (BMI ≤ 15th percentile). 22 variants were confirmed by Sanger sequencing. Genotyping was performed in up to 705 independent obesity trios (extremely obese child and both parents), 243 extremely obese cases and 261 lean adults.
Results and Conclusion
We detected 20 different non-synonymous variants, one frame shift and one nonsense mutation in the 7 continuous genomic regions in study groups of different weight extremes. For SNP Arg695Cys (rs58983546) in TBC1D1 we detected nominal association with obesity (pTDT = 0.03 in 705 trios). Eleven of the variants were rare, thus were only detected heterozygously in up to ten individual(s) of the complete screening sample of 372 individuals. Two of them (in FTO and MSRA) were found in lean individuals, nine in extremely obese. In silico analyses of the 11 variants did not reveal functional implications for the mutations. Concordant with our hypothesis we detected a rare variant that potentially leads to loss of FTO function in a lean individual. For TBC1D1, in contrary to our hypothesis, the loss of function variant (Arg443Stop) was found in an obese individual. Functional in vitro studies are warranted.
We identified the dsRNA binding protein RbdB as an essential component in miRNA processing in Dictyostelium discoideum. RbdB is a nuclear protein that accumulates, together with Dicer B, in nucleolar foci reminiscent of plant dicing bodies. Disruption of rbdB results in loss of miRNAs and accumulation of primary miRNAs. The phenotype can be rescued by ectopic expression of RbdB thus allowing for a detailed analysis of domain function. The lack of cytoplasmic dsRBD proteins involved in miRNA processing, suggests that both processing steps take place in the nucleus thus resembling the plant pathway. However, we also find features e.g. in the domain structure of Dicer which suggest similarities to animals. Reduction of miRNAs in the rbdB- strain and their increase in the Argonaute A knock out allowed the definition of new miRNAs one of which appears to belong to a new non-canonical class.
