Patienten mit JIA erhielten weniger Lebendimpfungen als gesunde Kontrollpersonen, für Totimpfstoffe waren die Immunisierungsraten vergleichbar.
Ebenso zeigten die Antikörper Konzentrationen zwischen den JIA Patienten und den gesunden Kontrollpersonen keine signifikanten Unterschiede. Bei Untersuchungen innerhalb der JIA Patienten Gruppe zeigte lediglich der Abstand zwischen der letzten Auffrischungsimpfung und der Blutentnahme signifikante Ergebnisse.
Intrakranielle Blutungen sind im Säuglingsalter seltene, aber lebensbedrohende Ereignisse. Neben Gefäßmissbildungen, Stoffwechseldefekten sowie Störungen der Blutgerinnung kommen v. a. nichtakzidentielle Traumata, Schütteltrauma in Betracht. Die klinische Diagnostik umfasst hinsichtlich der Blutungsgenese neben Sonographie und MRT als apparatives Verfahren auch eine Fundoskopie sowie laborchemische Analysen, insbesondere der Gerinnungsparameter. Für die Blutgerinnung ist das fettlösliche Vitamin K essenziell: Frühe, klassische und späte Vitamin-K-Mangel-Blutungen werden dabei unterschieden. Um ein gehäuftes Wiederauftreten von Vitamin-K-Mangel-Blutungen bei Neugeborenen und jungen Säuglingen zu verhindern, bedarf es einer hinreichenden Aufklärung der Eltern. Eine Verweigerung der Prophylaxe scheint Folge einer weltanschaulich begründeten Ablehnung der Schulmedizin und ein zunehmendes Phänomen in wohlhabenden Industrieländern zu sein.
Es wurden mögliche frühzeitige Immunoseneszenzparameter bei Kindern mit Autismus und ADHS im Vergleich zu gesunden Kontrollen mittels real-time quantitativer PCR ermittelt. Zusätzlich wurden T-Zell-spezifische Transkriptionsfaktoren mittels real-time quantitativer PCR gemessen. Es zeigte sich bei Autismus, im Vergleich zu gesunden Kontrollen, eine signifikant erniedrigte Anzahl von TRECs und eine signifikant erhöhte Expression des Th2-Zell-Transkriptionsfaktors GATA3. Schlußendlich scheint es eine schnellere Alterung des Thymus bei Kindern mit Autismus zu geben. Weitere Untersuchungen zur genaueren Differenzierung sind notwendig.
Common variable immunodeficiency (CVID) is the most common primary immunodeficiency in adults. It is associated with hypogammaglobulinemia, recurring infections and autoimmune phenomena. Treatment includes immunoglobulin substitution and immunosuppressants. Autoimmune neurological manifestations of CVID are rare and occur predominantly as granulomatous disease. We report the case of a 35-year-old woman with CVID who developed autoimmune encephalitis as demonstrated by double cerebral biopsy. Infectious or malignant causes could be excluded. Despite intensive immunosuppressive therapy with common regimens no significant improvement could be achieved. Ultimately, an autologous hematopoietic stem cell transplantation (HSCT) was performed, resulting in lasting complete remission of the encephalitis. To our knowledge, this is the first report of refractory autoimmune phenomena in CVID treated by autologous HSCT.
Mature dendritic cells (DCs) represent cellular adjuvants for optimal antigen presentation in cancer vaccines. Recently, a combination of prostaglandin E\(_2\) (PGE\(_2\)) with Toll-like receptor agonists (TLR-P) was proposed as a new standard to generate superior cytokine-producing DCs with high migratory capacity. Here, we compare TLR-P DCs with conventional DCs matured only with the proinflammatory cytokines TNFα and IL-1ß (CDCs), focussing on the interaction of resulting DCs with CD8\(^+\) T-cells. TLR-P matured DCs showed elevated expression of activation markers such as CD80 and CD83 compared to CDCs, together with a significantly higher migration capacity. Secretion of IL-6, IL-8, IL-10, and IL-12 was highest after 16 h in TLR-P DCs, and only TLR-P DCs secreted active IL-12p70. TLR-P DCs as well as CDCs successfully primed multifunctional CD8\(^+\) T-cells from naïve precursors specific for the peptide antigens Melan-A, NLGN4X, and PTP with comparable priming efficacy and T-cell receptor avidity. CD8\(^+\) T-cells primed by TLR-P DCs showed significantly elevated expression of the integrin VLA-4 and a trend for higher T-cell numbers after expansion. In contrast, TLR-P DCs displayed a substantially reduced capability to cross-present CMVpp65 protein antigen to pp65-specific T cells, an effect that was dose-dependent on PGE2 during DC maturation and reproducible with several responder T-cell lines. In conclu-sion, TLR-P matured DCs might be optimal presenters of antigens not requiring processing such as short peptides. However, PGE\(_2\) seems less favorable for maturation of DCs intended to process and cross-present more complex vaccine antigens such as lysates, proteins or long peptides.
Within the last decades, therapeutic advances, such as antenatal corticosteroids, surfactant replacement, monitored administration of supplemental oxygen, and sophisticated ventilatory support have significantly improved the survival of extremely premature infants. In contrast, the incidence of some neonatal morbidities has not declined. Rates of bronchopulmonary dysplasia (BPD) remain high and have prompted neonatologists to seek effective strategies of non-invasive respiratory support in high risk infants in order to avoid harmful effects associated with invasive mechanical ventilation. There has been a stepwise replacement of invasive mechanical ventilation by early continuous positive airway pressure (CPAP) as the preferred strategy for initial stabilization and for early respiratory support of the premature infant and management of respiratory distress syndrome. However, the vast majority of high risk babies are mechanically ventilated at least once during their NICU stay. Adjunctive therapies aiming at the prevention of CPAP failure and the support of functional residual capacity have been introduced into clinical practice, including alternative techniques of administering surfactant as well as non-invasive ventilation approaches. In contrast, the strategy of applying sustained lung inflations in the delivery room has recently been abandoned due to evidence of higher rates of death within the first 48 h of life.
This evidence‐based clinical guideline provides consensus‐recommendations for the treatment and care of patients with primary antibody deficiencies (PADs). The guideline group comprised 20 clinical and scientific expert associations of the German, Swiss, and Austrian healthcare system and representatives of patients. Recommendations were based on results of a systematic literature search, data extraction, and evaluation of methodology and study quality in combination with the clinical expertise of the respective representatives. Consensus‐based recommendations were determined via nominal group technique. PADs are the largest clinically relevant group of primary immunodeficiencies. Most patients with PADs present with increased susceptibility to infections, however immune dysregulation, autoimmunity, and cancer affect a significant number of patients and may precede infections. This guideline therefore covers interdisciplinary clinical and therapeutic aspects of infectious (e.g., antibiotic prophylaxis, management of bronchiectasis) and non‐infectious manifestations (e.g., management of granulomatous disease, immune cytopenia). PADs are grouped into disease entities with definitive, probable, possible, or unlikely benefit of IgG‐replacement therapy. Summary and consensus‐recommendations are provided for treatment indication, dosing, routes of administration, and adverse events of IgG‐replacement therapy. Special aspects of concomitant impaired T‐cell function are highlighted as well as clinical data on selected monogenetic inborn errors of immunity formerly classified into PADs (APDS, CTLA‐4‐, and LRBA‐deficiency).
Purpose of Review
To describe in detail the clinical synopsis and pathophysiology of chronic non-bacterial osteomyelitis and SAPHO syndrome.
Recent Findings
Chronic non-bacterial osteomyelitis (CNO) has been identified as a disease entity for almost 50 years. This inflammatory bone disorder is characterized by osteolytic as well as hyperostotic/osteosclerotic lesions. It is chronic in nature, but it can present with episodic flairs and phases of remission, which have led to the denomination “chronic recurrent osteomyelitis”, with its severe multifocal form “chronic recurrent multifocal osteomyelitis” (CRMO). For almost three decades, an infectious aetiology had been considered, since especially Propionibacterium acnes had been isolated from bone lesions of individual patients. However, this concept has been challenged since long-term antibiotic therapy did not alter the course of disease and modern microbiological techniques (including PCR) failed to confirm bone infection as an underlying cause. Over recent years, a profound dysregulation of cytokine expression profiles has been demonstrated in innate immune cells of CNO patients. A hallmark of monocytes from CNO patients is the failure to produce immune regulatory cytokines interleukin-10 (IL-10) and IL-19, which have been linked with genetic and epigenetic alterations. Subsequently, a significant upregulation of pro-inflammatory, NLRP3 inflammasome-dependent cytokines (IL-1β and TNF-α), has been demonstrated.
Summary
The current knowledge on CNO, the underlying molecular pathophysiology, and modern imaging strategies are summarized; differential diagnoses, treatment options, outcome measures, as well as quality of life studies are discussed.
Komplikationen und Langzeitfolgen nach hämatopoetischer Stammzelltransplantation in der Pädiatrie

Die allogene und autologe HSCT hat sich in den letzten 50 Jahren zu einer effektiven Therapiemethode für verschiedene maligne und nicht- maligne Erkrankungen entwickelt. Aufgrund der intensiven Therapieform ist die Behandlung mit Stammzellen mit einem hohen Risiko behaftet Komplikationen währen der Therapiedauer selbst aber auch spätere Langzeitkomplikationen zu erleiden. Eine ausschließlich nach der Übertragung allogener Fremdzellen auftretende Komplikation ist die akute oder chronische GvHD. In vorliegender Analyse werden aufgetretene Komplikationen und Langzeitfolgen, sowie mögliche Risikofaktoren für die Entstehung dargestellt. Untersuchte Patientengruppe besteht aus 229 pädiatrischen Patienten, die sich im Zeitraum vom 01.01.2005 – 31.12.2015 an der Universitäts- Kinderklinik Würzburg einer allogenen oder autologen HSCT unterzogen haben. Es wurden deskriptive und statistische Analysen in Bezug auf Patientencharakteristika, Stammzellcharakteristika sowie im Besonderen aufgetretene Komplikationen und Langzeitfolgen im Gesamtkollektiv sowie getrennt nach allogener und autologer HSCT und in Bezug auf die einzelnen Diagnosegruppen hin untersucht. Die Ergebnisse zwischen den Subgruppen wurden miteinander verglichen, um Risikofaktoren für das Auftreten von Komplikation und Langzeitfolgen in unserem Kollektiv zu ergründen. Bei 74,2% aller Patienten traten Komplikationen oder Langzeitfolgen auf. In Bezug auf die Transplantationsform allogen vs. autolog wurde ein signifikant höheres Risiko für Patienten nach allogener HSCT festgestellt. 77,0% (n=126) nach alloHSCT, sowie 53,4% (n=103) nach autoHSCT zeigten Komplikationen / Langzeitfolgen nach Stammzelltransplantation. Speziell in den Kategorien passagere (p=<.001) / chronische (p=.009) Infektionen, passagere respiratorische Insuffizienz (p=.005), therapiepflichtige Hyper/ - Hypotonien (p=<.001), Thrombosen/Embolien (p=.002), sowie pathologische Werte in nachfolgend durchgeführten Lungenfunktionstests (p=.043) haben alloHSCT- Patienten ein erhöhtes Risiko. Im Gegensatz zu den Komplikationen zeigen alloHSCT-Patienten jedoch höhere Zahlen der Patienten in Remission. 58,7% nach allogener Stammzellübertragung und 44,7% nach autologer HSCT zeigten sich am Ende des Beobachtungszeitraumes in Remission. In Bezug zu den Diagnosegruppen der Grunderkrankung zeigen Leukämiepatienten mit 77,9% innerhalb dieser Gruppe das höchste Risiko für Komplikationen und Langzeitfolgen nach HSCT. Als eine der schwerwiegendsten Komplikationen gilt GvHD. Risikofaktoren konnten hinsichtlich der HLA-Kompatibilität, dem Verwandtschaftsgrad des Spenders sowie durchgeführter Graftmanipulation detektiert werden. 57,3% der Patienten mit aGvHD, und 67,9% mit cGvHD erhielten Zellen von nicht verwandten Fremdspendern. Die Transplantate wurden in 69,0% mit nachfolgender akuten GvHD und in 82,1% mit chronischer GvHD vor Übertragung nicht bearbeitet (selektioniert / depletiert). Nach Datenanalyse zeigten signifikante Unterschiede zwischen den Transplantationsgruppen sowie auch zwischen den Diagnosegruppen. Risikofaktoren für sind einerseits die Art der Transplantation, verabreichte Medikamente in der Vortherapie sowie im Rahmen der Konditionierung, Bestrahlungen, Geschlecht und das Alter zum Zeitpunkt der Stammzelltransplantation spielen signifikante Rollen in der Entstehung von nachfolgenden Komplikationen und irreversiblen Organdysfunktionen.
Almost half of all preterm births are caused or triggered by an inflammatory process at the feto-maternal interface resulting in preterm labor or rupture of membranes with or without chorioamnionitis (“first inflammatory hit”). Preterm babies have highly vulnerable body surfaces and immature organ systems. They are postnatally confronted with a drastically altered antigen exposure including hospital-specific microbes, artificial devices, drugs, nutritional antigens, and hypoxia or hyperoxia (“second inflammatory hit”). This is of particular importance to extremely preterm infants born before 28 weeks, as they have not experienced important “third-trimester” adaptation processes to tolerate maternal and self-antigens. Instead of a balanced adaptation to extrauterine life, the delicate co-regulation between immune defense mechanisms and immunosuppression (tolerance) to allow microbiome establishment is therefore often disturbed. Hence, preterm infants are predisposed to sepsis but also to several injurious conditions that can contribute to the onset or perpetuation of sustained inflammation (SI). This is a continuing challenge to clinicians involved in the care of preterm infants, as SI is regarded as a crucial mediator for mortality and the development of morbidities in preterm infants. This review will outline the (i) role of inflammation for short-term consequences of preterm birth and (ii) the effect of SI on organ development and long-term outcome.
